4 Effect of Reserpine on the autophagy of rat aortic vascular smooth muscle cells

Objective To explore the relationship of Reserpine and the autophagy of rat aortic vascular smooth muscle cells (RA-VSMC). Methods RA-VSMC was cultured in medium with Reserpine or Earles balanced salt solution (EBSS) in vitro. MTT assay was used to detect the effect of Reserpine on the proliferation of RA-VSMC with 5 μmol/L Reserpine, 10 μmol/L Reserpine, 20 μmol/L Reserpine and 50 μmol/L Reserpine. The dual-fluorescence mRFP-GFP-LC3 was used to trace the autophagosomes of RA-VSMC. Real-time fluorescence quantitative PCR (qPCR) and Western blot (WB) were used to detect the expression of LC3 and Beclin1. Results The MTT results showed that compared with the control group, the cell viability in the 5 μmol/L Reserpine group, 10 μmol/L Reserpine group, 20 μmol/L Reserpine group and 50 μmol/L Reserpine group were significantly reduced (P < 0.05 or P < 0.01). The results of the dual-fluorescence mRFP-GFP-LC3 showed that there were no significant autophagosomes in the cells of the control group and the 10 μmol/L Reserpine group, and the 50 μmol/L Reserpine group and EBSS group formed significant autophagosomes. qPCR results showed that compared with the control group, there was no significant difference in LC3 mRNA expression level in the 10 μmol/L Reserpine group and the control group (P > 0.05). LC3 mRNA expression levels in 50 μmol/L Reserpine group and EBSS group increased significantly (P < 0.05 or P < 0.01). Beclin1 mRNA expression levels was significantly increased in 10 μmol/L Reserpine group, 50 μmol/L Reserpine group and EBSS group (P < 0.01). WB results showed that, compared with the control group, the 10 μmol/L Reserpine group, 50 μmol/L Reserpine group and EBSS group LC3-Ⅱ/LC3-Ⅰ ratio increased significantly (P < 0.01), Beclin1 protein expression was significantly increased in 10 μmol/L Reserpine group, 50 μmol/L Reserpine group and EBSS group (P < 0.01). Conclusion Reserpine may regulate the proliferation of vascular smooth muscle cells through autophagy.

8 Changes of sodium channel protein in intestine of functional diarrhea model rats with spleen deficiency

Objective To observe the changes of NHE3, Na+-K+-ATPase and ENaC in intestine of functional diarrhea model rats with spleen deficiency. Methods Twenty-four male Wistar rats (3 weeks old) were divided into normal group and model group according to the random number table method. The model rats of functional diarrhea with spleen deficiency syndrome were made using the modified multiple platform method and high lactose feed. After the model was made, the materials were taken and the expression of various indexes in intestinal tissue was detected by Western blot. Results The first day after molding, the weight of rats in both groups was significantly heavier than that in the first day before modeling (P < 0.01), but the weight of rats in model group was significantly lighter than that in normal group (P < 0.01). The diarrhea rate of rats in model group was 100%. After molding, the relative expression of NHE3 and Na+-K+-ATPase in colon of model group was lower than that of normal group (P < 0.05 or P < 0.01), the relative expression of NHERF2 and PKAR2 in colon of model group was higher than that of normal group (P < 0.05 or P < 0.01). Conclusion The down-regulation of Na+-K+-ATPase, NHE3 and other sodium channel proteins may be the molecular biological basis of spleen deficiency syndrome of functional diarrhea, which is worth further study and confirmation.

11 Mechanism prediction of Guishao Pingchan Formula against Parkinson′s disease based on IPA platform

Objective To explore the molecular mechanism of Guishao Pingchan Formula against Parkinson′s disease (PD) based on ingenuity pathway analysis (IPA) platform. Methods The chemical constituents of Guishao Pingchan Formula [Carapax et Plastrum Testudinis (Guijia), Radix Paeoniae Alba (Baishao), Rhizoma Ligustici Chuanxiong (Chuanxiong) and Rhizoma Gastrodiae (Tianma)] were searched from China national knowledge infrastructure (CNKI), viper database (VIP), China biology medicine disc (CBMdisc), Chinese materia medica, PubMed, Web of Science, SciFinder Scholar and active target proteins were retrieved from PubChem from April 1994 to December 2019. Meanwhile, PD related genes were retrieved from Gene. Finally, target proteins of Guishao Pingchan Formula and PD related genes were uploaded to IPA platform, and the molecular networks and classical biological pathways involved in them were compared. Results Three hurdred and twenty-three human active target proteins (58 Guijia, 119 Baishao, 72 Chuanxiong, 74 Tianma) and 392 PD related genes were obtained, then a major molecular network in which both were involved were built. Meanwhile, 10 main canonical pathways involved together were selected, such as G-protein coupled receptors signaling. Conclusion Guishao Pingchan Formula might play an anti-PD role by regulating G-protein coupled receptors signaling and other pathways.

16 Correlation analysis between heart rate variability and severity of sepsis

Objective To explore the correlation between heart rate variability and severity and prognosis of sepsis. Methods A total of 98 sepsis patients (observation group) admitted to People′s Hospital of Aba Tibetan and Qiang Autonomous Prefecture from January 2017 to January 2019 were selected, while according to acute physiology and chronic health evaluation (APACHE Ⅱ) score, the observation group was divided into sepsis group (0-10 scores, 39 cases), severe sepsis group (11-20 scores, 32 cases) and sepsis shock group (>20 scores, 27 cases). A total of 50 healthy patients (control group) in the same period were selected for dynamic electrocardiogram. Heart rate variability parameters (standard deviation of normal-to-normal intervals [SDNN], standard deviation of averaged normal-to-normal intervals [SDANN], root of the mean square of successive differences [rMSSD], percentage of adjacent normal-to -normal intervals differing by more than 50 ms [PNN50], low frequency [LF], high frequency [HF], LF/HF) were recorded. The heart rate variability parameters, APACHE Ⅱ score and prognosis were analyzed. Results The SDNN, LF, LF/HF in the observation group were lower than those in the control group (P < 0.05). The SDNN, LF, LF/HF in the severe sepsis group and septic shock group were lower than those in the sepsis group, while those in the septic shock group were lower than those in the severe sepsis group, and the differences were statistically significant (P < 0.05). SDNN, LF, LF/HF were negatively correlated with APACHE Ⅱ score (r = -0.459, -0.367, -0.405, P < 0.05). The area under the curve (AUC) of SDNN, LF, LF/HF to predict the prognosis of sepsis patients were 0.817, 0.802, and 0.937, respectively. The best cutoff values were 95.21 ms, 5.13 ms, and 0.41, while the sensitivity and specificity were 0.807 and 0.895; 0.645 and 0.881; 0.871 and 0.940, respectively. Conclusion Heart rate variability parameters SDNN, LF, LF/HF are closely related to the severity and prognosis of sepsis, which can be used as sensitive indicators for prognosis prediction.

20 Effect of the placental pathological changes of murine cytomegalovirus infected mice on the intrauterine growth and development of mice offspring

Objective To observe the efficacy of pathological placental changes after intrauterine murine cytomegalovirus (MCMV) infection on the intrauterine growth and development of fetal mice. Methods The mature mice without MCMV infection identified by enzyme-linked immunoabsorbent assay were served as the subjects. The male and the female mice were put into the same cage. According to the method of random number table, the mice were randomly divided into control group and model group, with 8 mice in each group. At day 12.5 of gestation, the placentas of the model group were injected with MCMV, while the control group was injected with cell maintenance medium. All female mice were executed at day 18.5 of gestation, and the change of tissues and organs of 98 fetal mice were observed. Results The placental infection rate and fetal infection rate in the model group were significantly higher than those in the control group, the differences were statistically significant (P < 0.05). In the placenta infection mice, the fetal infection rate increased significantly compared with those without placenta infection, the difference was statistically significant (P < 0.05). In the placenta infection mice, the rates of stillbirths and abnormal fetuses in mice with cytotrophoblast proliferative, syncytial trophoblast nodules and fibrinoid necrosis were significance higher than those without cytotrophoblast proliferative, syncytial trophoblast nodules and fibrinoid necrosis, the differences were statistically significant (P < 0.05). The teratogenic rate in mice with fibrinoid necrosis was significantly higher than those without fibrinoid necrosis, the difference was statistically significant (P < 0.05). Conclusion The MCMV infection through placenta is an important approach for intrauterine infection. MCMV infection is an important cause of adverse pregnancy outcomes in offspring.

24 Application value analysis of modified CarbaNP method and CIM method in detection of carbapenem-resistant antibiotics Acinetobacter baumannii and Pseudomonas aeruginosa

Objective To investigate the application value analysis of modified CarbaNP method and carbapenems enzyme inactivation method (CIM) in the detection of carbapenem-resistant antibiotics Acinetobacter baumannii and Pseudomonas aeruginosa. Methods A total of 120 strains of Acinetobacter baumannii and 100 strains of Pseudomonas aeruginosa were collected from Xingtai Third Hospital, Hebei Province from January 2014 to June 2015. Bacteria identification and drug susceptibility test were carried out with automatic microbiological instrument. Modified CarbaNP method and CIM method were used to detect bacterial carbapenems, and then gene test was used for validation. Results Among the 120 Acinetobacter baumannii strains, carbapenems antibiotic resistant strains and sensitive strains accounted for 61.67% and 38.33%, respectively. Among the 100 strains of Pseudomonas aeruginosa, carbapenems antibiotic resistant strains and sensitive strains accounted for 60.00% and 40.00% respectively. In Acinetobacter baumannii, the OXA-23 gene carrying rate was 94.59% (70/74), which was taken as the standard, and there was no significant difference between the modified CarbaNP method and the CIM method and the gene detection method (P > 0.05). The modified CarbaNP method and the CIM method were compared with the results of the genetic test respectively which showed that the sensitivity and specificity of the modified CarbaNP method and the CIM method were slightly higher than that of the modified CarbaNP method, but the differences were not statistically significant (P > 0.05). In Pseudomonas aeruginosa, the positive carrying rate of VIM-1 was 93.33% (56/60), which was taken as the standard, and there was no significant difference between the modified CarbaNP method and the CIM method and the gene detection method (P > 0.05). The modified CarbaNP method and the CIM method were compared with the results of the genetic test respectively which showed that the sensitivity and specificity of the modified CarbaNP method and the CIM method were slightly higher than that of the modified CarbaNP method, but the differences were not statistically significant (P > 0.05). Conclusion The modified CarbaNP method and CIM method have some value in the rapid detection of carbapenem-resistant Antibiotics acinetobacter baumannii and Pseudomonas aeruginosa, and the latter has better detection efficiency.

28 Screening of active parts of Simiao Yong′an Decoction based on PPAR-γ agonist and analysis its chemical composition

Objective To screen the active parts of Simiao Yong′an Decoction with peroxisome proliferators-activated receptor γ (PPAR-γ) activation and analyze its chemical components, and to explore the active components and anti-inflammatory effects of the extract. Methods Eight alcohol extracts of Simiao Yong′an Decoction were obtained by alcohol extraction and resin separation. The PPAR-γ was used as the target to screen the active parts of the extract of Simiao Yong′an Decoction at the cell level. A dose-dependent curve was drawn for the extracts with active tendency. EC50 value and relative activity value were calculated, and the chemical composition of active parts was analyzed and identified by liquid chromatograph-tandem mass spectrometer technology. Results In the PPAR-γ activation model, the extracts CS04 and CS05 have strong PPAR-γ activation. The chemical components of the active parts CS04 and CS05 with strong PPAR-γ agonistic effects were identified. A total of 31 components were identified. Conclusion CS04 and CS05 of Simiao Yong′an Decoction have strong PPAR-γ activation, which may be related to the inhibition of atherosclerosis inflammation.

34 Comparative study on the tyrosinase inhibitory effect of some ethanol extracts of traditional Chinese medicine

Objective To study the whitening activity of ethanol extracts of several traditional Chinese medicine, and to provide experimental basis for the development of whitening products of traditional Chinese medicine. Methods Nine traditional Chinese medicine including lesser galangal rhizome, common bletilla rubber, dahurian angelica root, sappan wood, largehead atractylodes rhizome, Radix Arnebiae, argy wormwood leaf, purslane herb, and medicine terminalia fruit were extracted with 70% ethanol. Vitamin C and arbutin were used as positive control drugs. In vitro tyrosinase inhibition test was employed to compare the bioactivity of different drugs. Results The inhibition rate of traditional Chinese medicine medicine terminalia fruit was (51.43±2.52)%, was higher than (46.34±6.32)% of arbutin. Artemisiae Argyi, Herba Portulacae also have good tyrosinase inhibity, the inhibition rates were (22.88±7.99)% and (13.07±11.98)%, respectively. However, the other Chinese medicine hardly inhibited tyrosinase activity. Conclusion The ethanol extracts of medicine terminalia fruit and argy wormwood leaf have certain inhibitory activities on tyrosinase, which can be further studied as whitening tradiational Chinese medicine.

37 Research progress of targeted intervention of traditional Chinese medicine in airway non-adrenergic non-cholinergic nerve in the treatment of asthma

The etiology of asthma is complex, and the pathogenesis can be summarized as airway immune-inflammation mechanism, neuromodulation mechanism and their interaction. Airway non-adrenergic non-cholinergic (NANC) nerves and the neurotransmitters they release can interfere with the onset of asthma by regulating airway smooth muscle contraction, mucus secretion, reversible airway obstruction, and airway hyperresponsiveness. Nerve growth factor affects the plasticity of NANC nerve by regulating the neurokinin released by NANC nerve, such as substance P. The syndrome differentiation of asthma in traditional Chinese medicine is “wheezing disease” and “gasp syndrome”. Current studies on the regulation of airway inflammation in asthma by traditional Chinese medicine are more detailed, while reports on the intervention of traditional Chinese medicine in the NANC nerve and its neurotransmitters in asthma are rare. In this paper, the intervention of traditional Chinese medicine on the NANC nerve and its neurotransmitters and related downstream pathways, as well as the influence of nerve growth factor on the NANC nerve are briefly reviewed in recent years, in order to broaden the thinking for clinical and experimental studies.

41 Advantages and development status of femtosecond laser small-incision lenticule extraction

As a new type of corneal refractive surgery, femtosecond laser small-incision lenticule extraction (SMILE) is popular among patients and surgeons because of its advantages of flapless, microinvasive, security, accuracy and stability. It becomes one of the mainstream operations of refractive surgery. At the same time, the corneal stromal lenticulere, which moved during the SMILE, has been found to be used in the treatment of diseases, such as keratoconus and hyperopia, as well as corneal stromal lenticule autoimplantation, lamellar keratoplasty, corneal stromal lenticule punctual plug, which has a unique clinical extension. However, SMILE still has some shortcomings, such as narrow application scope and limited retreatment. It needs further development and improvement. This article reviews both advantages, disadvantages and development status of SMILE treatment for myopia, astigmatism and hypermetropia, by comparing other methods of corneal refractive surgery.

45 Research progress of circulating miRNA in the treatment and prognosis of hepatocellular carcinoma

Hepatocellular carcinoma (HCC) was one of the common malignant tumors with a high mortality rate. Early and effective diagnosis of HCC was essential. MicroRNA (miRNA) was involved in the proliferation, differentiation, migration, neovascularization, apoptosis and other activities of liver cancer cells, which was expected to become new biomarkers to assist in the diagnosis, treatment and prognosis of liver cancer. This article mainly reviews the research progress of serum or plasma miRNA in the clinical application of liver cancer, and aimed to provide new ideas for the clinical diagnosis, treatment and evaluation of liver cancer prognosis.

Infertility is a common disease in gynecology. In recent years, the incidence of infertility caused by female ovulation disorders has increased year by year, and it is on the rise in the world, which has attracted the attention of the medical community. Modern clinical studies have shown that acupuncture has a good effect in treating ovulation disorders. It has obvious effects in promoting ovulation, improving clinical symptoms and enhancing clinical pregnancy rate, while has various treatment methods. It is easy to operate and does not affect the intrauterine environment. Therefore, this paper combines Chinese and foreign literatures in recent years to systematically review the research progress of acupuncture treatment of the disease from the aspects of etiology, pathogenesis and operation methods, while explore the ovulation of body acupuncture, plum blossom needle, and application factors (electroacupuncture, warm acupuncture). The effect of dysfunction of infertility, seeking a more complete treatment plan, providing scientific theoretical basis and standard clinical guidance for clinicians acupuncture treatment of ovulation disorders, and improving the scientific and effective treatment of ovulation disorders by acupuncture and moxibustion. Fully promote the national quintessence of Chinese to treat more infertility patients and benefit the people.

54 Recent advances in the rehabilitation treatment of post-stroke depression

Post-stroke depression (PSD) is a common complication after stroke, and also an important obstacle to the recovery of the disease. The incidence rate is high, which seriously affects the prognosis and quality of life of the patients. Therefore, the disease is deeply concerned by the clinic. At present, a large number of in-depth studies have generally believed that early detection and early treatment play a significant positive role in the recovery of the disease. The treatment methods are various of PSD, mainly Western medicine, other methods include psychological counseling, rehabilitation treatment, traditional Chinese medicine treatment, acupuncture and moxibustion treatment, etc., all of which have certain advantages. This paper summarizes the main methods of clinical treatment of the disease in the past five years, including Western medicine/traditional Chinese medicine treatment, psychological/emotional intervention, early rehabilitation training, physical therapy, acupuncture treatment, moxibustion treatment and other methods, in order to understand the progress of clinical rehabilitation treatment of PSD in China, deepen and standardize the understanding of systematic treatment of PSD, and provide reference for clinical treatment.

58 Research progress on preparations of naringin

Naringin is a natural flavonoid compound, which has the pharmacological effects of inhibiting the proliferation of thyroid cancer cells and inducing their apoptosis, protecting the damaged myocardium, etc. The conventional preparation of naringin has poor absorption and low bioavailability, which limits the clinical application of naringin. Studies have showed that naringin is prepared into naringin hydrophilic gel matrix tablets, naringin liposome gel, naringin-lecithin inclusion complex, naringin solid lipid nanoparticles, naringin nanoemulsions can improve bioavailability and enhance sustained release and targeting. Therefore, the development of new dosage forms is of great significance for expanding the clinical application of naringin. This study reviews the naringin preparations in recent years through literature retrieval, in order to provide references for the further research and development of naringin preparations.

62 Study on the priority of funds in local medical colleges to guarantee undergraduate education

With the development of science and technology and the progress of society, it is an important task to invigorate the country through science and education. Local medical colleges and universities should actively respond to the call of the state to actively develop first-class discipline construction. The quality of running a university can be measured by the use of educational funds. It is imperative to improve the use efficiency of education funds, give priority to the investment of undergraduate education funds, explore the financial management mode suitable for the characteristics of local medical colleges and establish a long-term guarantee mechanism. Based on the current situation of local medical colleges and universities, this paper explores the problems from the aspects of perfecting the system of per student appropriation, calculating the per student cost, making reasonable charges policy, strengthening the construction of financial internal control system management, finding out the existing problems and putting forward feasible suggestions.

66 The new trends of occupational health and occupational medicine and its enlighten to undergraduate education

The fast development of industrialization and informatization has caused remarkable changes in the distribution, working environment and health data of the occupational population. The objectives of occupational health and occupational medicine have also changed accordingly. New trends of the targets of occupational health and occupational medicine have appeared in the following aspects: the evaluation of occupational hazards, occupational health maintenance, work efficiency promotion and occupational health services. These changes put forward new requirements for the background and personal ability of the practitioners in this field. Great challenges have also been raised to the undergraduate education of occupational health and occupational medicine in universities. To meet these challenges, learning ability, multidisciplinary knowledge integration, information or big data processing, corporation, and the ability to do research should be emphasized during undergraduate training. By doing this, the students could be competent for future work in this field.

70 Preliminary study on the undergraduate teaching mode of clinical pharmacy based on the concept of “integration”

Compared with pharmacy, traditional Chinese medicine and pharmaceuticals, clinical pharmacy is a relatively new undergraduate specialty in medical colleges and universities in China. Therefore, the curriculum system, teaching means and methods of this undergraduate specialty are not mature enough and are still in the exploratory stage. By summarizing and reflecting on the teaching concept, teaching content and teaching methods of doctor of clinical pharmacy (Pharm.D.) of the University of Nebraska Medical Center (UNMC), the author puts forward the concept of “integration” into the teaching reform of clinical pharmacy specialty in the new period, and forms the rudiment of the reform opinions in all aspects of clinical pharmacy specialty teaching, to provide reference for the training of clinical pharmacy professionals in the future.

The emergence of the “biological-psychological-social” medical model has alleviated the current tension in medical treatment in China to some extent, and the smooth implementation of the new medical model has deeper demands for communication between doctors and patients. Good doctor-patient communication can not only promote doctors to better understand the patient′s condition, improve the efficiency of diagnosis and treatment, but also reduce the incidence of injuries and doctors, and promote the harmonious development of doctors and patients. As the main body of future medicine, clinical medical students should pay attention to and multi-pronged measures to strengthen the cultivation of their doctor-patient communication ability-help them strengthen communication awareness, control their emotions, improve theoretical basic knowledge from multiple disciplines, and adopt novel the teaching method continues to simulate practice, thereby strengthening language communication skills, achieving better doctor-patient communication, and improving the current tense doctor-patient relationship.

78 Analysis of 335 cases of severe patients transferred by ventilator outside the hospital

Objective To explore the mode, parameter conversion reference and application effect of outside the hospital transporter ventilator. Methods A total of 335 severe patients who need to use transporter ventilator as respiratory support were selected from May 2016 to May 2019 by first-aid personnel in the North District Branch, Beijing Emergency Medical Center as the subjects of this study. Changes in patients′ condition during the transfer process were observed, and changes in patients′ vital signs at the beginning and end of transfer were compared. Results Among the 335 severe patients were treated with transporter ventilator, only 19 patients had adverse events such as man-machine confrontation, which were not suitable for the reasons such as transporter ventilator or patient′s vital signs could not be maintained. Their families gave up the transfer and withdrew the vehicle on the spot. The rest of the patients were in stable condition on the way and successfully arrived at the destination. The transfer success rate was 94.3%. There were no statistically significant differences in heart rate, systolic blood pressure, diastolic blood pressure and blood oxygen saturation between the beginning of and the end of transfer (P > 0.05). Conclusion Refer to the ventilation mode and parameters of the hospital (or home) ventilator used by the patient on site, and set the ventilator ventilation mode and parameters of the transporter ventilator according to the actual situation of the patients, so that the patients can be transferred to hospital or sent to hospital with stable condition, obvious effect and safe delivery.

82 Clinical observation of Anisodamine Hydrobromide Injection on the excretion of small common bile duct stones

Objective To observe the effect of Anisodamine Hydrobromide Injection on the excretion of single symptomatic common bile duct (CBD) stones with diameter ≤10 mm. Methods A retrospective analysis was performed on the stone discharge of 127 patients with single symptomatic CBD stones with diameter ≤10 mm diagnosed in the Department of General Surgery, Xinjin Branch of the Sichuan Provincial People′s Hospital,  from September 2016 to June 2019. Among them, sixty-eight patients who received of Anisodamine Hydrobromide Injection were included in the experimental group. Fifty-nine patients who did not receive Anisodamine Hydrobromide Injection were treated as the control group. In the experimental group, Anisodamine Hydrobromide Injection was added on the basis of general treatment, 10 mg/time, intravenous injection, q6h, continuous use for 12 days. The control group received only general treatment. The discharge rate and adverse reactions of CBD stones in the two groups were compared. Results The discharge rate of 6 day and total discharge rate of CBD stones in the experimental group were higher than those in the control group, with statistically significant differences (P < 0.05 or P < 0.01). The discharge rate of CBD stones with diameter < 5 mm in the experimental group was higher than that in the control group, and the difference was highly statistically significant (P < 0.01). There was no statistically significant difference in the discharge rate of CBD stones with a diameter of 5-10 mm between the experimental group and the control group (P > 0.05). In the experimental group, the rate of promoting the discharge of CBD stones with a diameter of < 5 mm was higher than that of CBD stones with a diameter of 5-10 mm, and the difference was highly statistically significant (P < 0.01). There was no significant difference in self-discharge rate between the CBD stones with a diameter of < 5 mm and CBD stones with a diameter of 5-10 mm in the control group (P > 0.05). In the experimental group, 8 patients presented with flushing and dry mouth, but no other adverse reactions were observed. The difference of adverse reactions between the two groups was statistically significant (P < 0.05). Conclusion Anisodamine Hydrobromide Injection has the effect of promoting the discharge of single symptomatic CBD stones with the diameter ≤10 mm, especially for CBD stones with the diameter <5 mm, which is of good safety.

86 Clinical study of endovascular obstruction caused by tracheobronchial tuberculosis treated by bronchoscopy

Objective To investigate the clinical study of endovascular obstruction caused by tracheobronchial tuberculosis treated by bronchoscopy. Methods From May 2018 to May 2019, 380 patients with endovascular obstruction caused by tracheobronchial tuberculosis in Shenzhen Third People′s Hospital were selected. According to different treatment methods, they were divided into the observation group and the control group, with 190 cases in each group. The control group was treated with routine standard anti-tuberculosis treatment, while the observation group was treated with bronchoscopy on the basis of the control group. The sputum negative conversion rate, peripheral blood inflammatory factors, dyspnea degree, inner diameter of trachea and pulmonary function were compared between the two groups. At the same time, the complications and therapeutic effect of the two groups were evaluated. Results The sputum negative conversion rate of the observation group was significantly higher than the control group (P < 0.05). Compared with before treatment, the tumor necrosis factor-α, interleukin-6 and C-reactive protein decreased in the two groups after treatment, the observation group was lower than that in the control group (P < 0.05). Compared with before treatment, the dyspnea degree decreased of the two groups, and the observation group was lower than that of the control group (P < 0.05). Compared with before treatment, the inner diameter of trachea and the first second forced expiratory volume increased in the two groups, and the observation group was higher than those in the control group (P < 0.05). The total incidence of complications in the observation group was lower than that in the control group, and the total effective rate of treatment was higher than that in the control group (P < 0.05). Conclusion Endobronchial obstruction caused by tracheobronchial tuberculosis treated by bronchoscopy is reliable and safe, which is worthy of clinical application.

90 Effect of hydrocolloid dressing combined with wet hot compress on hematoma after arteriovenous fistula puncture

Objective To investigate the effect of hydrocolloid dressing combined with wet hot compress on hematoma after arteriovenous fistula puncture. Methods A total of 108 patients with hematoma after arteriovenous fistula in Central South University Xiangya School of Medical Affiliated Haikou Hospital from October 2016 to June 2018 were selected as the subjects, they were divided into the control group (54 cases) and the observation group (54 cases) by random number table method. The control group was given wet hot compress with 50% magnesium sulfate, and the observation group was given hydrocolloid dressing apply based on the control group. The hematoma regression time, blood flow, pain degree, degree of vascular sclerosis, time of ecchymosis disappearance and treatment effect were compared between the two groups. Results After 2 weeks of treatment, the hematoma regression time in the observation group was shorter than that in the control group (P < 0.05); blood flow in the two groups increased and the observation group was higher than that in the control group (P < 0.05); the degree of pain in the observation group was significantly lower than that in the control group (P < 0.05); the degree of vascular sclerosis in the observation group was lower than that in the control group (P < 0.05); the effective rate of treatment in the observation group was higher than that in the control group (P < 0.05). Conclusion Hydrocolloid dressing combined with 50% magnesium sulfate wet hot compress can significantly reduce the hematoma time, increase the blood flow of arteriovenous fistula, reduce the degree of pain and atherosclerosis, and promote the regression of ecchymosis and improve the treatment effect.

94 Study on the correlation between body mass index, blood lipids, insulin resistance and serum alarin in different glucose tolerance population

Objective To investigate the body mass index(BMI), blood lipids, insulin resistance (IR) and serum alarin levels among different glucose tolerance population. Methods A total of 124 people who underwent oral glucose tolerance test (OGTT) in Qinhuangdao Cerebrovascular Disease Hospital of Hebei Province from June 2017 to June 2019 were selected as the study subjects. According to the results of OGTT, they were divided into 43 cases in type 2 diabetes (T2DM) group, 36 cases in impaired glucose tolerance group and 45 people in normal group. The height and weight of the study subjects were measured, Blood lipids, serum alarin, fasting plasma glucose (FPG) and fasting insulin (FINS) were detected and BMI, homeostasis model insulin resistance index (HOMR-IR) and homeostasis model assessment-β (HOMA-β) were calculated. Results The differences of levels of alarin, BMI, triacylglycerol (TG), total cholesterol (TC), HOMR-IR and HOMA-β were statistically significant in three groups (P < 0.05). TC levels in T2DM group and impaired glucose tolerance group were lower than those in normal group and levels of alarin, BMI and TG were higher than those in normal group (P < 0.05). The levels of alarin and TG in T2DM group were higher than those in impaired glucose tolerance group (P < 0.05). The HOMR-IR in T2DM group and impaired glucose tolerance group were higher than those in normal group (P < 0.05), and T2DM group was higher than impaired glucose tolerance group (P < 0.05). HOMA-β of T2DM group was lower than that of impaired glucose tolerance group and normal group (P < 0.05). Serum alarin levels were positively correlated with BMI, TG and HOMR-IR (r =0.239, 0.367, 0.681, P < 0.05), and negatively correlated with HOMA-β (r = -0.323, P < 0.05). Conclusion Alarin, BMI, blood lipids and IR are different in different glucose tolerance population. Alarin is associated with BMI, blood lipids, and IR.

97 Researth on the effect of repetitive transcranial magnetic stimulation on SSI score of depression patients and its therapeutic effect

Objective To study the effect of repetitive transcranial magnetic stimulation (rTMS) on the score of Baker scale for suicidal ideation (SSI) in patients with depression and its therapeutic effect. Methods From September 2018 to June 2019, 112 cases with depressive patients who admitted to Ningbo Mental Hospital were selected as research subjects. They were divided into the control group and the experimental group according to the random number table method, with 56 cases in each group. The control group was treated with Escitalopram Oxalate, while the experimental group was treated with rTMS on the basis of the control group. The total effective rate, Hamilton depression scale-17 (HAMD-17) score, SSI score, the level of myelin basic protein (MBP) and brain-derived neurotrophic factor (BDNF) of the two groups were compared. Results The total effective rate of the experimental group was higher than that of the control group (P < 0.05). Before treatment, there were no significant difference in HAMD-17 and SSI scores between the two groups (P > 0.05). The HAMD-17 and SSI scores of the two groups after treatment were lower than those before treatment, and the experimental group was lower than the control group (all P < 0.05). There were no significant differences in serum MBP and BDNF levels between the two groups before treatment (P > 0.05). The serum MBP levels of the two groups after treatment were lower than those before treatment, and the experimental group was lower than the control group (P < 0.05). The levels of BDNF were higher than those before treatment, and the experimental group was higher than the control group (P < 0.05). Conclusion The effect of rTMS on depressive patients is significant, which can significantly alleviate depression, greatly reduce suicidal ideation, and effectively regulate the level of MBP and BDNF in serum.

101 The lung protective effect of Dexmedetomidine Hydrochloride in one lung ventilation of smoking patients undergoing lung cancer radical operation

Objective To investigate the lung protective effect of Dexmedetomidine Hydrochloride in one lung ventilation (OLV) of smoking patients undergoing lung cancer radical operation. Methods Forty smoking patients who underwent lung cancer radical operation in Jingmen First People′s Hospital in Hubei Province from September 2017 to September 2018 were selected as the study objects. According to the method of random number table, the patients were divided into control group and observation group, 20 cases in each group. In the observation group, 1 μg/kg Dexmedetomidine Hydrochloride was intravenously injected 10 min before anesthesia induction, and then 0.5 μg/(kg·h) was infused to 30 min before the end of operation. The control group was infused with the same volume of saline at the same speed. Before anesthesia induction (T0), immediately after OLV (T1), 60 min after OLV (T2), 30 min after recovery of bipulmonary ventilation (T3), and 2 h after operation (T4), the radial artery blood was taken, and the oxygenation index (OI) and respiratory index (RI) were calculated after the measurement of blood gas index; the concentrations of tumor necrosis factor-α (TNF-α) and intercellular adhesion molecule-1 (ICAM-1) were measured by enzyme-linked immunosorbent assay; the abnormal pulmonary function and the length of stay were recorded. Results Intra-group comparison: compared with T0, the control group at T2-T4 and the observation group at T2 and T3 the OI reduced and the RI increased significantly (P < 0.05); at T2-T4, the levels of TNF-α and ICAM-1 in the two group were significant increased (P < 0.05). Comparison between groups: compared with the control group, the OI of the observation group increased significantly and the RI decreased significantly at T2-T4 (P < 0.05); TNF-α and ICAM-1 levels in the observation group at T2-T4 significantly reduced (P < 0.05). Compared with the control group, the incidence of hypoxemia and pulmonary infection in the observation group was significantly reduced, and the length of stay was significantly shortened (P < 0.05). Conclusion Dexmedetomidine Hydrochloride can improve the ventilation and oxygenation function of the lung in OLV, protect the lung, reduce the incidence of inflammation and adverse reactions, and shorten the hospital stay.

105 Application of intravenous Lidocaine in painless fiberoptic bronchoscopy

Objective To investigate the effects of intravenous Lidocaine on the amount of Propofol during painless fiberoptic bronchoscopy and postoperative recovery. Methods From March 2019 to January 2020, a total of 100 patients undergoing painless fiberoptic bronchoscopy in the First Affiliated Hospital of Anhui Medical University were enrolled as the subjects. According to the random number table method, they were divided into control group and Lidocaine group, with 50 cases in each group. In the Lidocaine group, 1.5 mg/kg Lidocaine was intravenously injected during anesthesia induction, and the infusion was continued at a rate of 2 mg/(kg·h) until the end of the examination, while the control group was given the same amount of normal saline. The dose and total amount of Propofol induction, the changes of vital signs at different points during the perioperative period, the quality of recovery 40 (QoR-40) score 1 day before operation and 1 day after operation, the incidence of postoperative nausea and vomiting in the two groups were recorded. Results The total amount of Propofol in the Lidocaine group was lower than that in the control group, and the difference was statistically significant (P < 0.05). At T1 points, the heart rate (HR) and mean arterial pressure (MAP) of the control group were higher than those of T0 points, while the Lidocaine group were higher than those of the control group, and the differences were statistically significant (P < 0.05). In addition, HR and MAP interacted between groups (P < 0.05). At T1 and T2 points, pulse oxygen saturation in two groups were higher than those in T0 points, and the differences were statistically significant (P < 0.05). The score of mood state, physical comfort, pain and total score of the QoR-40 scale in the Lidocaine group on 1 d postoperatively were significantly higher than those in the control group (P < 0.05). The incidence of nausea and vomiting in the control group was significantly higher than that in the Lidocaine group, and the difference was statistically significant (χ2=5.01, P < 0.05). Conclusion Intravenous Lidocaine in painless bronchoscopy could reduce Propofol requirement and the incidence of nausea and vomiting after operation, and accelerate post-bronchoscopy recovery.

Objective To evaluate the application effect of sedation and analgesia with Fentanyl and Midazolam in modified percutaneous tracheostomy for severe patients in neurosurgery. Methods Clinical data of 71 patients whom underwent modified percutaneous tracheostomy in the First Affiliated Hospital of Hebei North University were collected from June 2018 to September 2019. They were divided into two groups according to the drugs for sedation and analgesia randomly, Fentanyl and Midazolam were used in the observation group (n = 36) and Diazepam or lytic cocktail were used in the control group (n = 35). The operation time, blood oxygen saturation, tracheal stimulus response, intraoperative and postoperative bleeding volume were compared between the two groups. Results There was no statistically significance in operation time of two groups (P > 0.05). There were no significant differences in intraoperative and postoperative bleeding volume of the two groups (P > 0.05). Tracheal stimulus response of observation group was weaker than control group, the difference was statistically significant (P < 0.01). Blood oxygen saturation in observation group was higher than that in control group, the difference was statistically significant (P < 0.01). No operation related death or pneumothorax case in two groups. Conclusion Sedative and analgesic therapy with Fentanyl and Midazolam can reduce tracheal stimulus response and maintain a stable blood oxygen saturation in modified percutaneous tracheostomy effectively, and it is a safer procedure.

112 Intervention research of Baduanjin exercise in relieving college students with mental fatigue

Objective To explore intervention effect of Baduanjin exercise in relieving college students with mental fatigue. Methods Sixty-six undergraduates majoring in medicine of Beijing University of Chinese Medicine were selected from May to August 2019 to participate in Baduanjin mental fatigue intervention experiment, and divided into intervention group (33 cases) and control group (33 cases) by random number table method. Intervention group began 8 weeks before the test and practiced 8 segments of brocade for 30 minutes in the morning every day. During the period, the final exam lasted for one week until the end of the exam, and the intervention lasted for 9 weeks. There was no intervention in control group. The subjects′ flash critical fusion frequency, fatigue scale (FS-14), job burnout scale and memory span were observed 8 weeks before and after test. Results In control group, the scores of the dimension of low achievement after test were lower than those 8 weeks before test, and the number of memory span test was higher than that 8 weeks before test, the differences were statistically significant (P < 0.05). After test, total scores of FS-14 fatigue, FS-14 mental fatigue, emotional exhaustion dimension and low achievement dimension in intervention group were lower than those 8 weeks before test, and the number of memory span test was more than that 8 weeks before test, the differences were statistically significant (P < 0.05). Total scores of FS-14 fatigue and emotional exhaustion dimension in intervention group were lower than those in control group, the differences were statistically significant (P < 0.05). Conclusion Persistent practice of Baduanjin has a good effect on alleviating mental fatigue of college students, which can provide college students an efficient and easy way to relieve mental fatigue caused by final exams and academic stress.

116 Study on correlation between damp heat constitution and inflammatory markers in obesity patients

Objective To explore the correlation between damp heat constitution and inflammatory markers in obese patients. Methods Eighty obesity outpatients from Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine and eighty normal weight physical examinees from January 2018 to June 2019 in Department of Preventive Medicine were selected and divided into four groups: obesity damp heat group (40 cases), obesity calmness group (40 cases), normal weight damp heat group (40 cases) and normal weight calmness group (40 cases). Body mass index, waist circumference, body fat rate, trunk fat rate, inflammatory markers (tumor necrosis factor α [TNF-α], interleukin-6 [IL-6], C-reactive protein [CRP] and leptin) and metabolic indexes were compared in each group. The correlation between inflammatory markers and the score of damp heat quality in obesity patients was analyzed. Results There was significant difference in the scores of damp heat quality among four groups (P < 0.05). There were significant differences in body mass index, waist circumference, body fat rate and trunk fat rate among four groups (P < 0.05). The body mass index, waist circumference, body fat rate and trunk fat rate of obesity damp heat group and obesity calmness group were all higher than those of normal weight damp heat group and normal weight calmness group (P < 0.05). The waist circumference, body fat rate and trunk fat rate of obesity damp heat group were higher than those of obesity calmness group (P < 0.05). The levels of TNF-α, IL-6, CRP and leptin among four groups were significantly different (P < 0.05). The levels of TNF-α in obesity damp heat group and obesity calmness group were higher than those in normal weight calmness group (P < 0.05). The level of TNF-α in obesity damp heat group was higher than that in obesity calmness group and normal weight damp heat group (P < 0.05). The levels of IL-6 and CRP in obesity damp heat group were higher than those in normal weight calmness group (P < 0.05). The levels of leptin in obesity damp heat group, obesity calmness group and normal weight damp heat group were higher than those in normal weight calmness group (P < 0.05). The levels of leptin in obesity damp heat group and obesity calmness group were higher than those in normal weight damp heat group (P < 0.05). The level of leptin in obesity damp heat group was higher than that in obesity calmness group (P < 0.05). The glycosylated hemoglobin (HbA1c) and triacylglycerol (TG) levels among four groups were significantly different (P < 0.05). HbA1c level of obesity damp heat group was higher than that of normal weight calmness group and normal weight damp heat group (P < 0.05). TG level of obesity damp heat group was higher than that of normal weight calmness group (P < 0.05). There was a positive correlation between TNF-α, IL-6, leptin and the score of damp heat quality in obesity patients (r = 0.37, 0.11, 0.62, P < 0.05). Conclusion There is correlation between damp heat constitution and inflammatory markers. The former may be a tool to evaluate the chronic inflammatory level of obesity patients.

Objective To investigate clinical efficacy of Yinqiao Powder combined with Maxing Shigan Decoction in the treatment of patients with acute exacerbation of chronic obstructive pulmonary disease (COPD). Methods One hundred and ninety-eight patients with acute exacerbation of COPD admitted to Jiaxing Hospital of Traditional Chinese Medicine (TCM) in Zhejiang Province from March 2015 to March 2018 were selected, they were divided into control group (n = 99) and study group (n = 99) according to the random number table method. Control group was treated with conventional Western medicine. Study group was treated with Yinqiao Powder and Maxing Shigan Decoction on the basis of control group. Clinical efficacy, TCM symptom score, pulmonary function, levels of inflammatory factors and incidence of adverse reactions were compared between two groups. Results After treatment, the scores of TCM syndromes in two groups were lower than those before treatment (P < 0.05), and those in study group were lower than those in control group (P < 0.05). Total clinical effective rate of study group was higher than that of control group (P < 0.05). After treatment, forced vital capacity (FVC) and forced expiratory volume in the first second (FEV1) of two groups were higher than before treatment, FEV1/FVC of two groups was higher than before treatment, and the improvement of study group was better than that of control group (P < 0.05). After treatment, the serum levels of interleukin-6, leukotrienes B4 and tumor necrosis factor-α in two groups were lower than those before treatment, and study group was lower than control group (P < 0.05). There was no significant difference in the incidence of adverse reactions between two groups (P > 0.05). Conclusion Yinqiao Powder and Maxing Shigan Decoction combined with routine Western medicine in the treatment of acute exacerbation of COPD can rapidly improve the clinical symptoms of patients with significant curative effect, effectively improve the pulmonary function of patients, reduce the level of inflammatory factors, drug safety is better, clinical application value is higher.

125 Effect of Qishen Yiqi Dropping Pills on long-term prognosis of patients with chronic heart failure

Objective To observe the effect of Qishen Yiqi Dropping Pills on long-term prognosis of patients with chronic heart failure. Methods Four hundred and eleven patients with chronic heart failure were selected from September 2017 to September 2018 in the First Central Hospital of Baoding City in Hebei Province. They were divided into control group (routine treatment, n = 206) and treatment group (routine treatment + Qishen Yiqi Dropping Pills, n = 205) by random number table method. Four weeks was a course of treatment, long-term use, follow-up time was 12-24 months (median follow-up time was 18 months). The levels of NT-proBNP, 6-MWT and LVEF were recorded between two groups before treatment, 6 and 12 months after treatment. Results Finally, 358 patients were included in the evaluation, including 182 cases in control group and 174 cases in treatment group. Overall analysis: there were significant differences in NT-proBNP, LVEF and 6-MWT between two groups with inter group comparison, time point comparison and interaction (P < 0.05). Further intra group comparison: there were significant differences in NT-proBNP, LVEF, 6-MWT between two groups before treatment, 6 and 12 months after treatment with comparison of time points (P < 0.05). Inter group comparison: there were no significant differences in NT-proBNP, LVEF and 6-MWT between two groups before treatment (P > 0.05). 6 and 12 months after treatment, the level of NT-proBNP in treatment group was lower than that in control group, LVEF in treatment group was higher than that in control group, 6-MWT in treatment group was longer than that in control group, the differences were statistically significant (P < 0.05). No serious adverse reactions were found in this study. Conclusion Qishen Yiqi Dropping Pills can obviously improve heart function and exercise tolerance of patients with chronic heart failure. It is worthy of clinical promotion.

129 Effect of Danshen Chuanxiongqin Injection combined with Low Molecular Weight Heparin in the treatment of pulmonary embolism

Objective To investigate the effect of Danshen Chuanxiongqin Injection combined with Low Molecular Weight Heparin Sodium in the treatment of pulmonary embolism. Methods Ninety-three patients with pulmonary embolism treated in Laibin People′s Hospital in Guangxi Zhuang Autonomous Region from March 2018 to September 2019 were selected, they were divided into control group (n = 46) and study group (n = 47) according to the random number table method. Control group was treated with Low Molecular Weight Heparin Sodium, study group was treated with Danshen Chuanxiongqin Injection on the basis of control group. Clinical efficacy of two groups after treatment was compared. Blood gas analysis indexes, D-dimer (D-D), troponin I (TnI), homocysteine (Hcy) of two groups were compared before and after treatment, and adverse reaction of two groups was recorded during treatment. Results Total effective rate of study group was higher than that of control group after treatment (P < 0.05). PaO2 of two groups after treatment was higher than that before treatment, which of study group was higher than that of control group (P < 0.05). After treatment, the levels of D-D, TnI, Hcy and PaCO2 in two groups were lower than those before treatment, and study group was lower than control group (P < 0.05). There was no significant difference in the incidence of adverse reactions between two groups (P > 0.05). Conclusion Danshen Chuanxiongqin Injection combined with Low Molecular Weight Heparin Sodium is effective in treating pulmonary embolism. It can effectively improve the serum level of TnI, Hcy, D-D and blood gas analysis indexes, without increasing the incidence of adverse reactions, and has certain clinical value.

133 Systematic evaluation and meta-analysis of Chengqi Decoction in the treatment of post-stroke consciousness disorder

Objective To assess the effects and safety of Chengqi Decoction in the treatment of post-stroke consciousness disorder. Methods CNKI, Wanfang, China biomedical literature database, PubMed, Embase, Cochrane Library database were searched by computer. It was included in the randomized controlled trial of Chengqi Decoction in the treatment of post-stroke consciousness disorder, with the retrieval time from the establishment of the database to April 2018. Two researchers independently selected literature, extracted data and evaluated the risk of bias, and RevMan 5.3 software was used to take meta-analysis. Results A total of 10 trails were enrolled, which included 775 cases. The meta-analysis showed that Glasgow coma score in experimental group was higher than that in control group (MD = 3.39, 95%CI[3.05, 3.73], P < 0.000 01). Differences also existed in subgroup analysis, differences on Glasgow coma score of acute ischemic stroke sub group (MD = 3.17, 95%CI [2.81, 3.53], P < 0.000 01), acute hemorrhagic stroke sub group (MD = 1.95, 95%CI [0.94, 2.97], P = 0.0002), oral sub group (MD = 5.65, 95%CI [3.84, 7.46], P < 0.000 01), nasal feeding sub group (MD = 3.29, 95%CI [3.16, 3.42], P < 0.000 01), enema sub group (MD = 1.80, 95%CI [0.94, 2.66], P < 0.0001), Huatan Tongfu sub group (MD = 5.65, 95%CI [3.84, 7.46], P < 0.000 01), Tongfu Kaiqiao sub group (MD = 3.27, 95%CI [3.10, 3.44], P < 0.000 01) were statistically significant. Conclusion On the basis of routine treatment, combined with Chengqi Decoction can better improve the level of consciousness of patients with post-stroke consciousness disorder, and the safety is better.

139 Usage analysis of oral Chinese patent medicine in Pediatric Outpatient of our hospital

Objective To understand the use of oral Chinese patent medicine in Pediatric Outpatient prescription of Beijing Fengtai District Maternal and Child Health Hospital (hereinafter referred to as “our hospital”), and analyze the existing problems and provide guarantee for strengthening the clinical rational and safe use. Methods Nine hundred and sixty prescriptions of oral Chinese patent medicine were randomly selected through hospital information system of our hospital from July 2018 to June 2019. According to the different ages of children, the use of drug action category, dosage form, toxic ingredients and unreasonable prescription were analyzed. Results There were 41 kinds of special oral Chinese patent medicines for children in our hospital, accounting for 19.34% of the total Chinese patent medicines in our hospital. In the prescription of oral Chinese patent medicine, action category and dosage form rank first were 329 pieces of antitussive, expectorant and antiasthmatic drugs (34.27%) and 18 kinds of granules (43.90%); there were 16 kinds of drugs containing toxic ingredients, including 14 kinds of oral drugs and 2 kinds of external drugs; there were 10 medicines containing bitter almonds; among them, there were 56 unreasonable prescriptions, accounting for the proportion of the prescriptions sampled 5.83%; the main problems were incomplete clinical diagnosis, inappropriate usage and dosage, inappropriate drug selection, etc. Conclusion The use of oral Chinese patent medicine in pediatrics of our hospital is generally reasonable, but because of the limitation of specialty, the Western medicine division is not reasonable in the use of Chinese patent medicine, so it is necessary to strengthen the training of Chinese medicine knowledge of Western medicine division to ensure that Chinese patent medicine is more reasonable and safe in the clinical use of pediatrics.

143 Application value of quantitative contrast-enhanced ultrasound in the early stage of chronic kidney disease

Objective To investigate the application value of quantitative contrast-enhanced ultrasound in early stage of chronic kidney disease (CKD). Methods A total of 51 cases of CKD patients who underwent kidney contrast-enhanced ultrasound in Xuanwu Hospital, Capital Medical University from August 2018 to October 2019 were selected as the study subjects. According to the clinical practice guidelines for CKD, They were divided into three groups, including 17 patients in group 1 (stage 1 CKD), 17 patients in group 2 (stage 2 CKD), and 17 patients in group 3 (stage 3-5 CKD). Kidney size, segmental renal artery peak systolic velocity (PSV),resistance index (RI), contrast-enhanced ultrasound parameters such as arrival time (AT), slope of ascending branches (A), time to peak (TTP), derived peak intensity (DPI), slope of descending branches α (α), area under the curve (AUC), and serum creatinine and urea nitrogen levels in three groups were analyzed. Results Serum creatinine and urea nitrogen levels in group 1 and group 2 were lower than those in group 3 (P < 0.05). There were no statistically significant differences in serum creatinine and urea nitrogen levels between group 1 and group 2 (P > 0.05). Cortical thickness and PSV of segmental renal artery in group 1 and group 2 were higher than those in group 3 (all P < 0.05), and there were no significant differences in cortical thickness and PSV of segmental renal artery between group 1 and group 2 (P > 0.05). DPI and AUC of group 1 were lower than those of group 2, and DPI and AUC of group 2 were higher than those of group 3 (all P < 0.05) and there were no significant differences in DPI and AUC between group 1 and group 3 (P > 0.05). Conclusion Quantitative renal contrast-enhanced ultrasound can be used for real-time monitoring of renal cortical blood perfusion, which can provide objective indicators for the assessment of renal function in patients with CKD. Especially in the early stage of CKD, the changes in CKD are more sensitive than those in common laboratory indexes such as blood creatinine and urea nitrogen, which is helpful for the early diagnosis and treatment of CKD in clinic.

147 Diagnostic value of acoustic radiation force impulse combined with fiber duct endoscopy in pathology nipple galactorrhea

Objective To investigate the value of acoustic radiation force impulse (ARFI) combined with fiber duct endoscopy (FDS) in diagnosis of pathology nipple galactorrhea. Methods A total of 35 patients (46 milk ducts) with nipple discharge from the Department of Ultrasound, Affiliated Hospital of North Sichuan Medical College from August 2017 to July 2018 with definite pathological results were selected as the study subjects. ARFI and FDS examinations were performed on the overflow side of the breast before surgery, ROC curve was drawn and the area under the curve (AUC) was recorded. Combined with the pathological results, the diagnostic value of ARFI and FDS in benign and malignant diseases of the nipple discharge side duct was compared. Results ARFI and FDS were helpful to distinguish the lesions. Virtual touch image quantification(VTIQ) of ARFI was most useful. When the critical value of VTIQ was 3.55, the sensitivity and specificity of predicting malignant tumors were 100.0% and 82.9%. The diagnostic accuracy of VTIQ combined with FDS were 91.3 %, and the misdiagnosis rate was 8.7%. However, compared with VTIQ and FDS, the difference was not statistically significant (P > 0.05). Conclusion Both ARFI and FDS are helpful to identify the tumor of pathological benign or malignant. ARFI combines with FDS can improve the diagnostic of accuracy, reduce the misdiagnosis.

151 Diagnostic value of contrast-enhanced ultrasound and elastography in cervical lymph node metastasis of elderly papillary thyroid carcinoma

Objective To evaluate the clinical application value of contrast-enhanced ultrasound and elastography in the diagnosis of cervical lymph node metastasis of elderly papillary thyroid carcinoma (PTC). Method A total of 117 elderly PTC patients (183 nodules) who admitted to Department of Special Diagnosis, 903rd Hospital of PLA from May 2015 to May 2017 were selected as the study objects. Contrast-enhanced ultrasound and ultrasound elastography were performed, and postoperative pathological results were used as the gold standard. According to whether PTC has lymph node metastasis, they were divided into metastasis group of 45 cases (74 nodules) and non metastasis group of 72 cases (109 nodules). Quantitative parameters, elastic strain rate and elastic score between the two groups were compared. The accuracy, specificity, sensitivity, negative and positive predictive value of contrast-enhanced ultrasound, elastography and their combination in the diagnosis of cervical lymph node metastasis of PTC were compared. The value of ultrasonography and elastography in the prediction of cervical lymph node metastasis of PTC were evaluated. Results The peak intensity of metastasis group was higher than that of non metastasis group, and the difference was statistically significant (P < 0.05). The elastic strain rate of metastasis group was higher than that of non metastasis group, and the difference was statistically significant (P < 0.05). SR predicted that the area under the curve of PTC cervical lymph node metastasis was greater than PI. The accuracy, sensitivity and negative predictive value of contrast-enhanced ultrasound combined with elastography in the diagnosis of cervical lymph node metastasis of PTC were higher than those of contrast-enhanced ultrasound or elastography alone. Conclusion Contrast-enhanced ultrasound and elastography have certain diagnostic value for cervical lymph node metastasis of PTC. The combination of them can effectively improve the diagnostic accuracy and has higher clinical application value.

156 Clinical effect of Kunxian Capsules combined with Valsartan in the treatment of diabetic nephropathy stage Ⅳ proteinuria

Objective To explore the clinical effect of Kunxian Capsules combined with Valsartan in the treatment of diabetic nephropathy stage Ⅳ proteinuria. Methods Sixty-one patients with diabetic nephropathy stage Ⅳ admitted to Tangshan Hospital of Traditional Chinese Medicine, Hebei Province from January 2018 to January 2019 were selected. According to the random number table method, they were divided into the control group and the observation group. Thirty patients in the control group were treated with Valsartan, and thirty-one patients in the observation group were treated with Kunxian Capsules on the basis of the control group. The treatment course of the two groups was 3 months. The changes of quantitative comparison of 24-hour urinary protein , renal function index(blood urea nitrogen [BUN], serum creatinine [Scr], endogenous creatinine clearance rate [Ccr]), liver function index (plasma albumin [Alb], alanine aminotransferase [ALT], aspartate aminotransferase [AST]) and peripheral blood leukocyte (WBC) of the two groups before and after treatment were observed and the adverse reactions were monitored. Results Before treatment, the quantitative comparison of 24-hour urinary protein between the two groups was not statistically significant difference (P > 0.05). After treatment, the quantitative comparison of 24-hour urinary protein in both groups was lower than that before treatment, and the observation group was lower than the control group, and the differences were statistically significant (P < 0.05 or P < 0.01). There were no statistically significant differences in BUN, Scr, Ccr, Alb, ALT, AST and WBC levels between the two groups before and after treatment (P > 0.05). No serious side effects were found that required drug withdrawal. Conclusion Kunxian Capsules combined with Valsartan is effective in the treatment of diabetic nephropathy stage Ⅳ, which can effectively reduce the level of patients with diabetic nephropathy proteinuria, so as to better protect renal function, and its safety is good, worthy of clinical promotion and application.

160 Cytogenetic analysis of sporadic chromosomal aberrations in peripheral blood

Objective To provide laboratory basis for genetic counseling and reproductive guidance through investigating the types and clinical manifestations of sporadic chromosomal aberrations in peripheral blood. Methods Peripheral blood chromosome examination specimens detected in the Reproductive Hospital of Guangxi Zhuang Autonomous Region from January 2015 to November 2019 were retrospectively analyzed. Relevant medical records and types of aberrations of patients with sporadic chromosomal aberrations were collected for genetic analysis. Results A total of 129 cases (1.5%) of sporadic chromosomal aberrations were detected in the specimens of 8367 subjects, and the type of aberration was mainly translocation. Among them, t(7; 14) was the most common (15.5%), and 62 cases (48%) were correlated with abortion. Conclusion The clinical significance of sporadic chromosomal aberrations cannot be ignored. Prenatal diagnosis should be recommended for people with such chromosomal aberration in their reproductive actions.

167 Application of intelligent nursing information system in emergency treatment of Chest Pain Center

Objective To analyze the application of intelligent nursing information system in emergency treatment of Chest Pain Center. Methods A total of 42 patients with acute ST elevation myocardial infarction (STEMI) who were admitted to the Department of Emergency of Shanghai Jiao Tong University Affiliated Chest Hospital (“our hospital” for short) from September 2016 to September 2017 were selected as the observation group to implement information-based nursing process intervention in Chest Pain Center. While a total of 40 patients with STEMI admitted to the Department of Emergency of our hospital from August 2015 to August 2016 were selected as the control group to implement the traditional emergency nursing path. The pre-examination and triage time, the first electrocardiogram time, the open venous access time, the oral administration time, the bedside point-of-care test (POCT)+ the test report time of the two groups were compared; the STEMI recurrence rate and the second percutaneous coronary interventional therapy (PCI) operation rate and ST segment reduction rate of the two groups were recorded; the emergency care satisfaction of the families of the two groups were compared. Results The time of pre-examination and triage, the time of the first electrocardiogram, the time of open venous access, the time of oral administration and the time of bedside POCT+ test report in the observation group were significantly shorter than those in the control group and the differences were highly statistically significant (P < 0.01). The treatment time of the observation group was shorter than that of the control group, while the success rate of first aid was higher than that of the control group, and the differences were statistically significant (P < 0.05). The STEMI recurrence rate and secondary PCI rate in the observation group were lower than those in the control group, and the ST segment reduction rate was higher than that in the control group, and the differences were statistically significant (P < 0.05). The satisfaction of the observation group (97.62%) was higher than that of the control group (77.50%), and the difference was statistically significant (P < 0.05). Conclusion The intelligent information system can effectively optimize the rescue nursing process of the chest pain center, shorten the treatment time of patients, improve the success rate of treatment, and enhance the satisfaction of nursing, which is worthy of promotion.

171 Effect of systematic standardized management mode intervention on self-behavior management and quality of life of diabetic patients

Objective To explore the effect of systematic standardized management mode intervention on self-behavior management and quality of life of diabetic patients. Methods A total of 260 diabetic patients admitted to Department of Endocrinology, Liaoning Provincial People′s Hospital from January 2017 to December 2019 were selected. They were divided into control group and observation group, with 130 cases in each group, according to the random number table method. The control group adopted the conventional management mode, while the observation group implemented the systematic standardized management mode. Blood glucose levels, summary of diabetes selfcare activities (SDSCA) scores and diabetes specific quality of life (DSQL) scores were compared between the two groups. Results After intervention, the levels of fasting blood glucose, 2 h postprandial blood glucose and glycosylated hemoglobin in the two groups were lower than those before intervention, while those in the observation group were lower than those in the control group, with statistically significant differences (P < 0.05). After the intervention, SDSCA scale scores, such as diet management, exercise therapy, blood glucose monitoring, foot care and drug treatment, while DSQL scale scores, such as physiological function, psychological/spiritual dimension, social relationship dimension and treatment dimension in the two groups were higher than those before the intervention, while those in the observation group were higher than those in the control group, with statistically significant differences (P < 0.05). Conclusion The application of systematic standardized management mode intervention can effectively stabilize and reduce blood glucose level, improve self-behavior management state, and enhance quality of life of patient.

175 Effect of time effect incentive intervention mode on the recovery of patients with cerebral hemorrhage after operation

Objective To explore the effect of time effect incentive intervention mode on the recovery of patients with cerebral hemorrhage after operation. Methods From June 2018 to June 2019, a total of 128 patients with intracerebral hemorrhage were selected and divided into observation group and control group by random number table method, with 64 cases in each group. The control group underwent routine intervention, while the observation group implemented a time-sensitive incentive intervention model. Self-efficacy, exercise compliance, quality of life, and national institute of health stroke scale (NIHSS) scores were compared between the two groups. Results After 3 months of intervention, the general self-efficacy score of the two groups were significantly higher than those before the intervention, while the observation group was higher than the control group, and the differences were statistically significant (P < 0.05). The observation group exercise compliance was higher than control group, and the difference was statistically significant (P < 0.05). The environmental adaptability, physiological function, social function, psychological function, and total score of the two groups were significantly higher than those before the intervention, while those in the observation group were higher than those in the control group, and the differences were all statistically significant (all P < 0.05). At 10 days after surgery, the NIHSS scores of the two groups were significantly lower than those before the intervention, while the observation group was lower than that in the control group, and the differences were statistically significant (P < 0.05). Conclusion The time-dependent incentive intervention model can improve self-efficacy and exercise compliance, improve quality of life and promote neurological recovery after cerebral hemorrhage surgery. It is worthy of promotion.

Objective To explore the effect of auricular point pressing (hereinafter referred to as “ear patch”) on perioperative pain of retinal detachment (RD). Methods A total of 64 cases of RD were diagnosed in the Department of Ophthalmology, Guangdong Provincial Hospital of Chinese Medicine from March to December 2018, and they were divided into the experimental group and the control group according to the random number table method, with 32 cases in each group. The control group was given routine analgesia and nursing, while the experimental group was given ear stickers 1 d before surgery on the basis of the control group, and the ear points were pressed once before, during and after 1 h. The changes of Visual analogue scale (VAS) score, short-form of McGill pain questionnaire (SF-MPQ) score, blood pressure and heart rate during the perioperative period were observed in the two groups. Results Postoperative and intraoperative VAS scores and SF-MPQ scores of the two groups were higher than those before operation, while the experimental group was lower than the control group, and the differences were statistically significant (P < 0.05). The intraoperative systolic blood pressure was higher than preoperative, postoperatively lower than preoperative, and the experimental group was lower than the control group, and the differences were statistically significant (P < 0.05). The intraoperative and postoperative heart rate was lower than before operation, while the experimental group was lower than the control group after operation, and the differences were statistically significant (P < 0.05). There were interactions between VAS score, SF-MPQ score, systolic blood pressure, heart rate in time and groups (P < 0.05). Conclusion Ear patch can relieve perioperative pain in RD patients, maintain blood pressure and heart rate relatively stable, and improve quality of life of patients during perioperative.

182 A qualitative study on the needs of home care service for disabled elderly patients after stroke

Objective To discuss the needs of home care for disabled elderly patients after stroke, and to provide reference for relevant departments to formulate practical home care policies and measures. Methods Based on the phenomenological research methods, 15 elderly patients with post-stroke disability who were admitted to the Department of Neurology of a Third Class Hospital in Shanghai from January to April 2019 were interviewed by objective sarapling. Colaizzi analysis method was used to analyze, summarize and refine the topics of the interview data. Results The interview revealed four topics: the need for knowledge of stroke disease, the need for continuous nursing services, the need for home-based rehabilitation and nursing, and the need for family and social support. Conclusion Elderly patients with post-stroke disability have a high demand for home-based nursing services, it is necessary to accelerate and improve the home-based nursing services for elderly patients with post-stroke disability, so as to promote rehabilitation, reduce family burden and improve their quality of life.

186 Application of operating room nursing combined with ward nursing in patients with severe head injury

Objective To explore the application effect of operating room nursing combined with ward nursing in patients with severe head injury. Methods From June 2014 to June 2015, 126 patients with severe craniocerebral injury were admitted to the Operating Room of the First Affiliated Hospital of Harbin Medical University, the patients were divided into the control group (from June to December 2014) for routine nursing intervention, the observation group (from January 2015 to June 2016) carried out the nursing mode of operation room nursing and ward nursing, with 63 cases in each group. After nursing, the quality of emergency care (pre-hospital emergency response time, emergency department treatment time, patient prognosis and the inicdence of transport accident) was compared between the two groups. Nursing satisfaction was observed in both groups. Results The pre-hospital emergency response time and emergency department treatment time of the observation group were shorter than those of the control group, and the difference was statistically significant (P < 0.05). The cure rate and nursing satisfaction in the observation group were higher than those in the control group, and the mortality rate and the incidence of transport accidents in the observation group were lower than those in the control group, with statistically significant differences (P < 0.05). Conclusion In patients with severe craniocerebral injury, the nursing mode of operation room nursing and ward nursing can improve the quality of first-aid nursing and nursing satisfaction.

190 Effect of continuous nursing on quality of life in patients with chronic obstructive pulmonary disease complicated with respiratory failure

Objective To analyze the effect of continuous nursing on the quality of life of patients with chronic obstructive pulmonary disease (COPD) and respiratory failure. Methods A total of 120 COPD patients with respiratory failure who were treated in Hebei Province Chest Hospital from December 2017 to December 2018 were selected as the research subjects. They were divided into two groups using the single and double number method, and the single number was the control group. The double number is the observation group, with 60 cases in each group. The control group used conventional care, the observation group added continuous care on the basis of conventional care. The two groups of patients were compared with the life impact before and after intervention (using the St.George′s respirator questionnaire [SGRQ]) and physical health (using 12 general health questionnaires [GHQ-12] assessment), body mass index (BMI), health knowledge mastery score. Results Before the intervention, there were no significant differences in the SGRQ score, GHQ-12 score, BMI, and health knowledge mastery score between the two groups (all P > 0.05). After the intervention, the SGRQ score and GHQ-12 scores of the two groups were lower than before the intervention, and the BMI and health knowledge mastery scores of the two groups were higher than before the intervention, the differences were statistically significant (all P < 0.05), and the SGRQ and GHQ-12 scores in the observation group were significantly lower than those of the control group, and the BMI and health knowledge mastery scores of the observation group were significantly higher than those of the control group, and the differences were statistically significant (all P < 0.05). Conclusion Continuing care has a significant impact on the quality of life of patients with COPD and respiratory failure, and it is worth promoting.

193 Application of hierarchical chain nursing method in pediatric asthma nebulizer therapy

Objective To explore the application of hierarchical chain nursing method in pediatric asthma nebulizer therapy. Methods Nursing data of 80 patients given pediatric asthma nebulizer therapy in Department of Pediatrics, Huzhou Maternal and Child Health Hospital from January 2016 to January 2018 was retrospectively analyzed. Depending on the type of care, the children given routine care were included in the control group (n = 35), children with hierarchical chain care nursing were included in the observation group (n = 45) on the basis of the control group. The therapeutic effect, symptom improvement, treatment adherence and satisfaction with care were compared between the two groups. Results The total effective rate of the observation group was higher than the control group (P < 0.05). The treatment compliance of the observation group remained unchanged after the treatment was prolonged, while the treatment compliance of the control group was poor after the treatment was delayed. Moreover, the compliance of the two groups at the second month was statistically significant (P < 0.05). The compliance of the two groups at 3-6 months was compared, and the difference was highly statistically significant (P < 0.01). The family members satisfaction scores in terms of hospitalization environment, nursing methods, health education, psychological intervention, nursing attitude and responsibility of the observation group were significantly higher than those of the control group (P < 0.05). Conclusion The use of hierarchical chain nursing in the treatment of pediatric asthma nebulizer can significantly improve the therapeutic efficacy, compliance and nursing satisfaction, improve the clinical symptoms of children, reduce the pain of children, it is worth promoting.
