7 Differential expression of microRNAs in serum and hairs different position of cutaneous melanoma

Objective To investigate the differential expression of microRNAs in cutaneous malignance melanoma in order to provide a basis for the diagnosis and targeted treatment of this malignancy; to screen seven specific microRNAs in serum and hairs of cutaneous malignancy melanoma. Methods From March 2016 to October 2017, 12 cases of cutaneous melanoma (CM)  confirmed by routine histopathology and immunohistochemistry in Shanghai Changzheng Hospital, Navy Military Medical University, were collected as CM group, while 17 cases of non-neoplastic skin diseases were collected as control group. The expression situation of microRNAs in serum and hair of two groups were detected by qRT-PCR. Candidate microRNAs with consistent results were identified as significant common differential expression biological indicators. The differentially expressed microRNAs (> 2.5 times) were screened by the multiple of differences between groups. Results There were 2 up-regulated and 5 down-regulated microRNAs in serum among 7 microRNAs of patients in CM group. The up-regulated microRNAs included miR-4487 and miR-4706, the down-regulated microRNAs included miR-16, miR-211, miR-4731, miR-509-3p and miR-514a. There were 3 up-regulated and 4 down-regulated microRNAs in hairs, the up-regulated microRNAs included miR-211, miR-4487 and miR-4731; the down-regulated microRNAs included miR-16, miR-4706, miR-509-3p and miR-514a. The co-up-regulated microRNAs in serum and hairs of the CM group were microRNAs-4487, but there was no statistically significant difference between the two groups (P > 0.05). Compared with the control group, the expressions of has-microRNA-16 and has-microRNA-509-3p were significantly down-regulated, the differences were statistically significant (P < 0.05), and with high consistency. Compared with control group, the expression of miRNA-541a in serum and hairs of CM group was down-regulated, but there was no statistically significant difference (P > 0.05). MiRNAs with more than 2.5 times differential expression in serum and hairs were miR-16 and miR-514a. In hair alone, miR-4706 was with differential expression of more than 2.5 times. The miR-4731 was more than 4 times differentially expressed in serum samples alone. MiRNAs with more than 10 times differential expression in serum were miR-211 and miR-509-3p. Conclusion Compared with non-cancer patients, there are many differentially expressed microRNAs in serum and hairs. There are highly significant differentially expressed microRNAs 211 and 509-3p in serum of CM patients, which can be used as biological markers of the tumor. Differential expression of microRNAs 4706 in hair samples may be the biology of early noninvasive detection of melanoma. Indicators. Detection of microRNAs is an available biological indicator for CM, and may become a means of target gene therapy in the future.

12 Effect of acupuncture for the treatment of alcohol withdrawal anxiety in rats and its relationship with CRH

Objective To observe and explore the effect and mechanism of acupuncture for the treatment of alcohol withdrawal anxiety in rats. Methods Fifty-four SPF healthy male SD rats were selected and randomly divided into blank group, model (alcohol withdrawal anxiety model) group, sānyīnjiāo group, zúsānlǐ group, electro-acupuncture two-point (sānyīnjiāo + zúsānlǐ) group, acupuncture two-point (sānyīnjiāo + zúsānlǐ) group, with 9 rats in each group. Except the blank group, all the other groups were given continuous intraperitoneal injection of alcohol for 28 days to prepare the alcohol addiction model, withdrawal of alcohol for 3 days to prepare the alcohol withdrawal anxiety model, and they were respectively given acupuncture at corresponding points during withdrawal of alcohol for 3 days. The elevated plus-maze (EPM) test was used to measure the anxiety behavior. The enzyme-linked immunosorbent assay (ELISA) was used to measure the concentration of corticotropin releasing hormone (CRH) in the hippocampus. The real-time fluorescence quantitative PCR was used to detect the expression of CRH mRNA in hippocampus. Results Compared with the blank group, the percentage of number and residence time of the model group staying in the open arms were significantly reduced (P < 0.05), the concentration of CRH in the hippocampus was significantly increased (P < 0.01), the expression of CRH mRNA was significantly increased (P < 0.01); compared with the model group, the percentage of number of entering the open arm and residence time staying in the open arm during EPM test of the electro-acupuncture two-point group were significantly increased (P < 0.05 or P < 0.01), the percentage of number entering the open arm during EPM test of the acupuncture two-point group was significantly increased (P < 0.05), CRH concentrations in hippocampus of the electro-acupuncture two-point group and the acupuncture two-point group were significantly decreased (P < 0.01), and CRH mRNA expression in hippocampus was also significantly decreased (P < 0.05 or P < 0.01). However, the effect of single acupoint group on each monitoring index was not significant (P > 0.05). Conclusion Acupuncture can relieve the anxiety of rats with alcohol withdrawal anxiety, and its anti-anxiety effect may be related to the regulation of CRH expression in the hippocampus of rats.

17 Expression of JAK2-STAT3 signaling pathway in lung tissues of rats with severe acute pancreatitis and its correlation with peripheral inflammatory factors

Objective To investigate expression of JAK2-STAT3 signaling pathway in lung tissues of rats with severe acute pancreatitis (SAP) and its correlation with peripheral inflammatory factors. Methods 5-month-old male Wistar rats (n = 240) were randomly divided into SAP group, SAP+R group, SAP+S group, SAP+R+S group and sham-operated group (SO group), 48 rats in each group. Abdominal aortic blood and lung tissues were collected 3, 6, 12 and 18 hours after model establishment in each group animals. Serum interleukin-6 (IL-6) and interleukin-10 (IL-10) were measured. The expression levels of JAK2 mRNA, STAT3 mRNA, p-JAK2 Protein and p-STAT3 protein in lung tissues were detected. Results The levels of serum IL-6 and IL-10 at 3, 6, 12 and 18 hours after model establishment were significantly different among the five groups (P < 0.05). There was significant difference in serum IL-10 level in SO group (P < 0.05). The serum levels of IL-6 and IL-10 in SAP group, SAP+R group, SAP+S group and SAP+R+S group increased gradually at 3 hours after model establishment, peaked at 12 hours after model establishment, and then decreased, there were statistically significant differences among different time points with in the group (P < 0.05). The expression levels of JAK2 mRNA, STAT3 mRNA, p-JAK2 protein and p-STAT3 protein in lung tissue of five groups at 3, 6, 12 and 18 hours after model establishment were significantly different (P < 0.05). The expression levels of JAK2 mRNA, STAT3 mRNA, p-JAK2 protein and p-STAT3 protein protein in lung tissues of five groups increased gradually at 3 hours after model establishment, peaked at 12 hours after model establishment, and then decreased, there were statistically significant differences among different time points within groups (P < 0.05). Conclusion Activation of JAK2/STAT3 pathway may be closely related to systemic inflammatory response in the course of SAP.

22 The expression and relationship between NgR and p-Tau in brains of senescence accelerated mice

To investigate the expression of NgR and p-tau in senescence accelerated mouse P8 (SAMP8) was studied by using Nogo receptor (NgR) antagonist NEP1-40, explore the possible role of NgR. Methods Six months old senescence accelerated mouse-resistence 1 (SAMR1) male mice were the control group (group A); 3 months old SAMP8 mice (group B). Sham treatment group (group C) and treatment group (group N) of SAMP8 mice at 6 months of age, with 6 mice in each group. Mice in group C were intraperitoneally injected with 0.2 mL 0.9% normal saline, and mice in group N were intraperitoneally injected with 0.2 mL NEP1-40 for 14 days. Morris water maze test was used to monitor behavioral changes in mice before and after treatment, and immunohistochemical staining was used to detect changes in p-Tau and NgR expression in nerve cells. Results Compared with group A, the incubation periods of water maze test in group B, C and N were significantly prolonged, and the times of crossing platform were significantly reduced (P < 0.05). Compared with the first water maze experiment, the incubation period of group N was significantly shortened and the times of crossing platform were significantly increased (P < 0.05). The NgR positive cell count and optical density of the hippocampus in group C were significantly increased compared with those in group A and group N (P < 0.05). The deposition of p-Tau positive cells in group C was significantly reduced compared with that in group A and group N(P < 0.05). Conclusion The learning and memory ability of SAMP8 mice decreased with the aging of the month, and NgR and p-Tau increased significantly in the hippocampus, which was consistent with the aging degree of the mice. NgR antagonist NEP1-40 may improve the learning and memory ability of mice by down-regulating NgR expression and reducing downstream p-Tau expression.

26 The role of microglia cells in cerebral ischemia reperfusion injury

Neuroinflammatory reaction plays an important role in the pathophysiology of cerebral ischemia-reperfusion injury. Microglia is an important immune effector cell in the central nervous system (CNS), and it is the first defensive line for CNS. Microglia can release a variety of inflammatory factors and a variety of growth factors and increase the expression of various proinflammatory enzymes. It plays a key role in the neuroinflammatory reactions. During cerebral ischemia-reperfusion injury occurs, microglia cells are activated by a variety of cytokines at“rest”, such as γ-interferon, tumor cytokine-α, colony stimulating factor, transforming growth factor, etc. Then it transforms into different phenotypes of “activated” microglia, namely M1 and M2 type. They play a dual effect of anti-inflammatory and pro-inflammatory, and affect the occurrence and development of cerebral ischemia-reperfusion injury. This review describes the physiological characteristics of microglia, factors affecting microglia activation, phenotypic transformation characteristics of microglia and factors regulating phenotypic changes, in order to understand the role of microglia in cerebral ischemia reperfusion injury, and characteristics of activation and phenotypic transformation. It provides the basis for the prevention, treatment and prognosis of cerebral ischemia-reperfusion injury.

29 Research progress of the compatibility related to Hewei Anshen Decoction in the treatment of insomnia

Insomnia is closely related to gastrointestinal dysfunction. According to the theory of “gastric disorder causing restless sleep”, the prescription of Hewei Anshen is used to treat insomnia, the clinical and experimental studies have shown that the curative effects of Hewei Anshen Decoction on insomnia is remarkable. Hewei Anshen Decoction is composed of ginger processed pinellia, spine date seed, coix seed, tuber fleeceflower stem, silktree albizia bark, grassleaf sweetflag rhizome, roast milkwort root and other traditional Chinese medicine. This paper reviews the research progress on the compatibility of ginger processed pinellia with spine date seed, ginger processed pinellia with coix seed, silktree albizia bark with tuber fleeceflower stem, roast milkwort root with grassleaf sweetflag rhizome and other traditional Chinese medicine, to explore the mechanism of Hewei Anshen Decoction in treating insomnia.

33 Progress in the application of drug-coated balloon in the treatment of in-stent restenosis

After percutaneous coronary intervention, in-stent restenosis is the most common complication in patients with coronary atherosclerosis, and its exact pathogenesis is unclear. The emergence of drug-eluting balloon provids a new method for coronary artery intervention treatment. However, there is still a lack of consensus on clinical application and prognostic evaluation, and especially the application effect of coronary in-stent restenosis is controversial. This article discusses the clinical application of drug-coated balloons in the treatment of coronary in-stent restenosis.

37 Progress of hand function rehabilitation in brain remodeling

In addition to traditional rehabilitation and occupational therapy, new technologies such as brain-computer interface, functional electrical stimulation, biofeedback therapy, transcranial magnetic stimulation, mirror therapy, movement imaging and robotics have emerged. The rehabilitation mechanism of these new technologies lies in establishment and recovery with the cerebral cortex remodeling. Rehabilitation of hand function is the key to upper limb rehabilitation after central nervous system damage. The process of hand function recovery is actually a process of remodeling brain function. Rehabilitation of hand function should be carried out as soon as possible with different personalized rehabilitation, so that patients can return to society as soon as possible.

In this paper, we review the progress of the empowerment education application status at home and abroad and intervention effect from the aspects of the concept of empowerment, in order to provide reference for application of empowerment education in type 2 diabetes management in China.

45 Research progress on external therapy of traditional Chinese medicine in treating knee osteoarthritis

Knee osteoarthritis (KOA) is one of the most common forms of arthritis, which can cause pain and joint deformity in the elderly. With the aging of the population, the incidence of KOA is increasing year by year, and its treatment methods are varied. External treatment of traditional Chinese medicine has achieved good results in the treatment of KOA and has certain advantages. In order to understand the latest advances in the treatment of KOA by external treatment of traditional Chinese medicine, this paper extensively searches Chinese databases such as CNKI, Wanfang database and Vip database, and searches domestic clinical research literature on external treatment of KOA in the past five years.

49 Research progress in percutaneous balloon compression for trigeminal neuralgia

Trigeminal neuralgia is one of the common diseases in functional neurosurgery. At present, there are various treatment methods for trigeminal neuralgia, and more and more scholars propose that percutaneous balloon compression has certain curative effect on the treatment of trigeminal neuralgia. In addition, percutaneous balloon compression is the only surgical treatment for trigeminal neuralgia with the least trauma, the shortest time and the highest comfort under general anesthesia. In this paper, the development history, postoperative complications and surgical methods of percutaneous balloon compression in the treatment of trigeminal neuralgia were reviewed to provide theoretical basis for clinical treatment.

54 The practice and new development of multidisciplinary team in the diagnosis and treatment of breast cancer

Breast cancer is one of the malignant tumors threatening to female health. The patient-centered multidisciplinary team (MDT) that gathers multidisciplinary experts to work together and formulates individualized treatments is the best management model for tumor diagnosis and treatment. MDT plays an important role in accurate diagnosis of breast cancer, decision-making, follow-up management, and research on new technologies. However, this model is still an exploratory stage in China and needs to be further improved. An efficient MDT team can not only help improve medical staff, but also improve patients′ survival rate and quality of life.

57 Spatial-temporal distribution of syphilis and its influencing factors in China (excluding Hong Kong, Macao and Taiwan) from 2010 to 2016

Objective To explore spatial-temporal distribution characteristics of syphilis epidemic situation in China (excluding Hong Kong, Macao and Taiwan) from 2010 to 2016 and its social influencing factors. Methods Spatial aggregation of syphilis epidemic situation in China (excluding Hong Kong, Macao and Taiwan) from 2010 to 2016 was analyzed by using geographic information software ArcGIS and spatial data analysis software GeoDa. Spatial data processing software SaTScan was used to retrospectively analyze syphilis epidemic situation from 2010 to 2016. Stata was used to carry out panel data regression analysis of syphilis epidemic situation. Results Global spatial autocorrelation analysis showed that there was no spatial aggregation of syphilis incidence in China (excluding Hong Kong, Macao and Taiwan) from 2011 to 2015 (Moran′s I = 0.0444 to 0.1099, P > 0.05), however, in 2010 (Moran′s I = 0.2524, P < 0.05) and 2016 (Moran′s I = 0.1932, P < 0.05), syphilis incidence in China (excluding Hong Kong, Macao and Taiwan) was spatially aggregated. The results of local autocorrelation analysis showed that Jiangsu, Shanghai and Fujian were in the high-high region in 2010, Gansu was in the low-high region in 2013-2016, and all regions in China (excluding Hong Kong, Macao and Taiwan) were not in the high-high region in 2011-2012. Retrospective spatio-temporal scanning analysis revealed that there was one primary clustering region (Shanghai, Zhejiang, RR = 2.49) and six secondary clustering regions (Guangxi, Xinjiang, Chongqing, Liaoning, Inner Mongolia and Ningxia, RR = 1.44-3.50). The results of panel data regression analysis showed that per capita GDP and consumption level were the influencing factors of the incidence of syphilis (P < 0.05). Conclusion The epidemic situation of syphilis has obvious characteristics of spatial-temporal aggregation and is affected by social demographic factors. Discussing the characteristics of spatial-temporal aggregation of syphilis can provide a positive reference for syphilis control strategy and evaluation of the effect of prevention and control measures.

64 Construction of post competency index system for digestive specialist nurses

Objective To explore the post competence of digestive specialist nurses, and to construct a scientific and reasonable post competence index system, so as to provide scientific basis for the evaluation of their competence level, and provide reference basis for the training, assessment, recruitment and post performance management of digestive specialist nurses. Methods From November 2018 to June 2019, using post competency theory as reference, the paper drew up an evaluation index framework for the competency of digestive nurses and prepared the inquiry paper of expert letters based on methods as document analysis, focus group interview and personal in-depth interview. After two rounds of expert consultation using Delphi method, the index was increased, decreased and modified by combining boundary value method and expert opinions. The post competency model of digestive specialist nurses was established after determination of index weight by percentage weight method, which was also demonstrated and improved by digestive nursing experts. Results The established model the post competency of digestive specialist nurses consisted of 3 first-level indexes, 9 second-level indexes and 39 three-level indexes, whose weight were determined. The three first-level indexes were in order of weight: clinical practical ability of digestive nursing (0.3548), professional accomplishment (0.3330), and comprehensive ability (0.3122). Conclusion The competency model of digestive specialist nurses constructed in this study is scientific and reasonable, and the model has passed the demonstration of experts in this field, which will provide reference basis for  training, examination, recruitment, and position performance management of digestive specialist nurses.

70 Relationship between resting heart rate at admission and serum inflammatory factors and brain natriuretic peptide in patients with chronic heart failure

Objective To investigate relationship between resting heart rate at admission and serum inflammatory factors and brain natriuretic peptide (BNP) in patients with chronic heart failure (CHF). Methods One hundred and fifty-three cases of CHF treated in Beijing Ditan Hospital, Capital Medical University from May 2013 to February 2018 were selected as subjects of study. According to the different resting heart rates at admission, patients were divided into three groups: group A, group B and group C. Group A (resting heart rate ≤70 beats/min) was 46 cases, group B (resting heart rate >70-<90 beats/min) was 52 cases and group C (resting heart rate ≥90 beats/min) was 55 cases. The resting heart rate, left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD) and left ventricular ejection fraction (LVEF) were measured under resting condition after the patients were admitted and had 1 hour rest. Fasting venous blood was collected on the next day after admission. Serum levels of C-reactive protein (CRP), BNP, interleukin (IL)-1β, IL-6 and tumor necrosis factor (TNF)-α were measured. Pearson correlation between resting heart rate at admission and serum inflammatory factors and BNP in patients was analyzed. Results The contents of CRP, BNP, IL-1β, IL-6 and TNF-α in group C were significantly higher than those in group A and B (P < 0.05), and the contents of the above indexes in group B were significantly higher than those in group A (P < 0.05). LVEDD and LVESD in group C were significantly longer than those in group A and B, and LVEF in group C was significantly lower than those in group A and B (P < 0.05); LVEDD and LVESD in group B were significantly longer than those in group A, and LVEF in group B was significantly lower than that in group A (P < 0.05). The number of NYHA class Ⅱ cases in group C was significantly less than that in group A and B, and the number of NYHA class Ⅲ/Ⅳ cases in group C was significantly more than those in group A and B (P < 0.05). Resting heart rate of CHF patients at admission was positively correlated with CRP, BNP, IL-1β, IL-6, TNF-α, LVEDD and LVESD (r = 0.350, 0.584, 0.305, 0.411, 0.349, 0.334, 0.395, all P < 0.05), and resting heart rate of CHF patients at admission was negatively correlated with LVEF (r = -0.425, P < 0.05). Conclusion The resting heart rate at admission is positively correlated with CRP, IL-1β, IL-6, TNF-α inflammatory factors and serum BNP levels in CHF patients, and is correlated with cardiac function.

74 Effect of Erchen Decoction combined with Taohong Siwu Decoction in treating hemorrhagic apoplexy caused by phlegm and blood stasis in convalescence

Objective To investigate the effect of Erchen Decoction and Taohong Siwu Decoction in treating hemorrhagic apoplexy caused by phlegm and blood stasis in convalescence. Methods From January 2014 to December 2017, 90 cases of hemorrhagic stroke convalescence patients admitted to Xi′an Encephalopathy Hospital of Traditional Chinese Medicine were selected. Patients were divided into control group (n = 45) and treatment group (n = 45) according to the random number table method. The control group received conventional treatment, the treatment group was treated with Erchen Decoction and Taohong Siwu Decoction on the basis of the control group. The two groups received continuous treatment for 28 days. The clinical efficacy of the two groups was evaluated. TCM syndrome score and Glasgow proghostic scale (GOS) score were measured before and after treatment, meanwhile, serum levels of hypersensitive C-reactive protein (hs-CRP), tumor necrosis factor-α (TNF-α) and interleukin-4 (IL-4) were measured before and after treatment and the incidence of adverse reactions in the two groups was observed. Results The clinical effective rate of the treatment group was significantly higher than that of the control group, and the difference was statistically significant (P < 0.05). After treatment, TCM syndrome score and serum hs-CRP, TNF-α and IL-4 levels in the treatment group were significantly lower than those in the control group (all P < 0.05); GOS scores were significantly higher than those of the control group, with statistically significant differences (P < 0.05). During the treatment, there was no statistically significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Erchen Decoction and Taohong Siwu Decoction has significant clinical effect in treating patients with phlegm and blood stasis blocking collaterals syndrome of hemorrhagic stroke during recovery, which can significantly improve clinical symptoms and reduce serum inflammatory factor level, and has good safety.

78 Effect of abdominal massage combined with ultrasonic therapeutic apparatus on gastrointestinal function and nutritional status of severe neurosurgical patients

Objective To analyze the effect of abdominal massage combined with ultrasonic therapy on gastrointestinal function and nutritional status of patients with severe neurosurgery. Methods A total of 88 cases of severe neurosurgical patients admitted from June 2016 to June 2018 in Nanfang Hospital of Southern Medical University were selected, patients were divided into control group and observation group by random number table method, with 44 cases in each group, the control group was only given peritoneal massage intervention, and the observation group was added to the control group with ultrasonic therapy. After treatment, gastrointestinal dysfunction scores, gastrointestinal function indicators, nutritional status and the satisfaction of patients′ family members were compared between the two groups. Results After intervention, the scores of gastrointestinal dysfunction in the two groups were lower than those before intervention (P < 0.05), and the score in the observation group was lower than that in the control group (P < 0.05). The effective rate of defecation in the observation group was higher than that in the control group (P < 0.05). After intervention, the levels of albumin, serum albumin, transferrin, hemoglobin and total cholesterol in the two groups were increased compared with those before intervention, and those in the observation group were higher than the control group (P < 0.05). The family satisfaction of the observation group was higher than that of the control group (P < 0.05). Conclusion The intervention of abdominal massage combined with ultrasound therapy for severe neurosurgical patients can effectively relieve their gastrointestinal dysfunction and improve their nutritional status, and is easy to be accepted by patients′ families.

82 Application of trigeminal somatosensory evoked potentials in early diagnosis of blepharospasm

Objective To investigate the value of trigeminal somatosensory evoked potential (TSEP) in the early diagnosis of blepharospasm. Methods From November 2016 to June 2017, 73 cases with early non-spasmodic symptoms of blepharospasm admitted to the Third People's Hospital of Dalian were selected and 30 cases of healthy people as group Ⅱ and groupⅠ respectively. According to the testing results of TSEP, patients of group Ⅱ were further divided into two subgroups, 40 cases (group Ⅱa) with abnormal TSEP and 33 cases (group Ⅱb) with the normal TSEP. The peak-to-peak amplitude of N13-P19, P19-N30, N13/P19 and P19/N30 at locus C (5,6), F (5,6) peak-to-peak amplitude at sites N13′-P19′, P19′- N30′, N13′/P19′ /N30′ in each group were statistically analyzed. One year of follow-up, the incidence of blepharospasm group Ⅱa and group Ⅱb were observed. Results Compared with group Ⅰ, P19/N30 peak-to-peak amplitude and P19′/N30′ peak-to-peak amplitude of goup Ⅱ were increased, the differences were statistically significant (P < 0.05), the other indicators showed no statistically significant difference (P > 0.05). Compared with group Ⅱb, P19/N30 peak-to-peak amplitude and P19′/ N30′ peak-to-peak amplitude of group Ⅱa were increased, the differences were highly statistically significant (P < 0.01), the other indicators showed no statistically significant difference (P > 0.05). One year of follow-up, the incidence of blepharospasm of groupⅡa was higher than group Ⅱb, the difference was statistically significant (P < 0.05). Conclusion TSEP can detect increased excitability of nerve nucleus group in the early stage of blepharospasm, providing basis for early diagnosis of blepharospasm, and can be popularized in clinical work.

0bjective To investigate the role of decision tree model and Logistic regression model in postpartum urinary retention (PUR) influencing factors analysis. Methods From January 1, 2014 to December 31, 2017, 180 puerpera with PUR after vaginal delivery in Lishui People′s Hospital of Zhejiang Province ("our hospital" for short) were selected as case group, and 200 puerpera who without PUR in our hospital at the same period were selected randomly as control group. Decision tree model and Logistic regression model were used to determine influential factors for PUR. Results Decision tree model and Logistic regression model indicated that delivery analgesia (P = 0.047), forceps delivery (P = 0.001) and episiotomy (P < 0.001) were independent risk factors of PUR, and fetal macrosomia (P = 0.023) was also the influencing factor in the Logistic regression model, but the decision tree model did not indicate its influence. Conclusion There are many factors influencing PUR. Decision tree model and Logistic regression model are complementary to each other, which can describe the factors from different aspects, and provide basis and reference for the further formulation of preventive measures.

89 Expression of fucoidyltransferase 4 in cervical cancer and its relationship with prognosis after interventional therapy

Objective To investigate the clinical significance of expression of fucoidyltransferase 4 (FUT4) in cervical cancer and its relationship with prognosis after interventional therapy. Methods Seventy-four cases of cervical cancer (cervical cancer group) in Hu′nan Maternal and Child Health Care Hospital from February 2012 to February 2017 were selected and treated with interventional therapy combined with hysterectomy, and 40 cases of hysterectomy with hysteromyoma were selected as control (control group), the expression of FUT4 protein was detected by Western blot, and the prognosis of patients was followed up. Results The relative expression of FUT4 protein in cervical cancer cervical cancer group was significantly higher than that in the control group (P < 0.05). The relative expression of FUT protein in patients with tumor diameter > 4 cm and lymph node metastasis were significantly higher than the tumor diameter ≤ 4 cm and no lymph node metastases (P < 0.05). The median duration of disease free progression and the total survival time in down regulation of FUT protein expression patients were higher than those in patients with the invariant and up-regulated expression of FUT protein (P < 0.05). Conclusion FUT4 protein is highly expressed in cervical cancer patients, which is related to the clinicopathological characteristics of patients. Patients with down-regulation of FUT4 protein after interventional therapy has a good prognosis.

93 The treatment effect of hysteroscopy combined with laparoscope in infertility patients

Objective To investigate the treatment effect of hysteroscopy combined with laparoscope in infertility patients. Methods From January 2015 to March 2018, 156 cases of infertility patients admitted to Wuzhou People′s Hospital were randomly selected and grouped according to the treatment methods and patients′ treatment intention, 78 cases in each group. Hysteroscopy combined with laparoscopy exploratory surgery was applied in combined group, laparoscopy surgery alone was applied of single group. Duration of operation, intraoperative blood loss, hospital stays, postoperative tubal recanalization rate, intrauterine pregnancy rate 1 year after surgery and delivery situation of the two groups were analyzed and compared. Results There was no statistically significant difference in duration of operation, intraoperative blood loss and hospital stays between the two groups (P > 0.05). Postoperative tubal recanalization rate and intrauterine pregnancy rate 1 year after surgery in combined group were higher than those in single group, the differences were statistically significant (P < 0.05). The rate of caesarean section and artificial midwifery in combined group was lower than that in single group, the vaginal delivery rate in combined group was higher than that in single group, the differences were statistically significant (P < 0.05). Conclusion The clinical effect of hysteroscopy combined with laparoscope treatment for infertility is significant. It can effectively improve the postoperative tubal recanalization rate and intrauterine pregnancy rate, and can conducive to improve the vaginal delivery rate, meanwhile reduce the artificial midwifery rate, it has a positive significance to improve the pregnancy outcome.

96 Clinical effect of ovale forceps combined with Chanhou Zhuyu Capsules to prevent late postpartum hemorrhage

Objective To explore the clinical effect of ovale forceps combined with Chanhou Zhuyu Capsules to prevent late postpartum hemorrhage. Methods From March 2016 to March 2019, 90 cases of caesarean section and spontaneous delivery women with decidual tissue residue in the uterine cavity and placenta examined were selected as the research objects. They were divided into the observation group and the control group according to random number table method, 45 cases in each group. The control group was treated with Chanhou Zhuyu Capsules orally, while the observation group was given oval forceps for clamping and clearing residual decidual tissue on the basis of the control group. The incidence rate of late postpartum hemorrhage, the amount of haemorrhagic lochia, the duration of haemorrhagic lochia, the height of uterine floor and the incidence of adverse reactions were compared between the two groups. Results The incidence rate of late postpartum hemorrhage of the observation group was lower than that of the control group, the difference was statistically significant (P < 0.05). The amount of haemorrhagic lochia in the observation group was less than that in the control group, and the duration of haemorrhagic lochia in the observation group was shorter than that in the control group, the differences were statistically significant (P < 0.05). The uterine floor height of the two groups at 7 d after treatment was lower than that at 4 d after treatment, and the uterine floor height of the observation group at 4 and 7 d after treatment was lower than those of the control group, the differences were statistically significant (P < 0.05). There was no statistically significant difference in the total incidence rate of adverse reactions between the two groups (P > 0.05). Conclusion Oval forceps for clamping and clearing residual decidual tissue combined with Chanhou Zhuyu Capsules to prevent late postpartum hemorrhage has a significant clinical effect, which can lower the incidence rate of late postpartum hemorrhage, reduce the bloody lochia, and shorten the duration of bloody lochia, promote postpartum recovery, and do not increase adverse reactions.

100 Effect of Apatinib in the treatment of recurrent extensive small cell lung cancer after multi-line chemotherapy and its impact on the survival of patients

Objective To observe the short-term efficacy, safety and survival of Apatinib in the treatment of patients with extensive small cell lung cancer (SCLC) who had previously undergone multiline chemotherapy. Methods A retrospective study of 62 SCLC patients treated with Apatinib in Department of Radiotherapy and Chemotherapy, Tangshan People′s Hospital from January 2017 to June 2018. The efficacy was evaluated for the first time after 1 cycle (4 weeks). Thereafter, the efficacy was evaluated every two cycles. The primary endpoint was progression-free survival (PFS) and the secondary endpoint was total survival (OS). Kaplan-Meier curve was used to evaluate PFS. Log-rank was used to compare survival differences between groups. The efficacy was evaluated according to RECIST 1.1 criteria, and adverse reactions were evaluated according to CTCAE 4.0 criteria. Results Among the 62 patients with SCLC, there were no CR cases, 11 cases (17.7%) with PR, 37 cases (59.7%) with SD and 14 cases (22.6%) with PD. DCR was 48 cases (77.4%). The median PFS was 3 months (95%CI: 1.13-4.87) and the median OS was 6 months (95%CI: 4.27-7.73). The most common adverse reactions were secondary hypertension (28/62) and hand-foot syndrome (34/62), and grade 3 adverse reactions occurred in 3 cases. Conclusion Apatinib has a definite effect in the third or fourth line treatment of extensive SCLC, and the adverse reactions are tolerable. The results need to be validated in a multi-center, large-sample prospective study.

104 Application of circRNA expression profile for the diagnosis of gastric cancer:a Meta-analysis

Objective To explore the value of circRNA expression profile in the diagnosis of gastric cancer by meta-analysis. Methods Chinese and English literatures on circRNA used for gastric cancer diagnosis were searched from Datebase of Wanfang, CNKI, PubMed and EMBASE from the time of building database to November 30, 2018. The quality of the literature was evaluated by quality assessment of diagnostic accuracy studies-2 (QUADAS-2); the overall data was combined using Stata 12.0; sensitivity analysis and meta-regression analysis were conducted to explore the sources of heterogeneity; circRNA expression profile used for the diagnosis of gastric cancer was detected by Deek′s funnel plot. Results A total of 12 eligible articles were enrolled. The total area under the curve (AUC) for diagnosis of gastric cancer by circRNA expression profile was 0.80. Corresponding sensitivity, specificity, diagnostic odds ratio (DOR) and 95%CI were 0.69 (95%CI: 0.66-0.71, P = 0.000), 0.73 (95%CI: 0.70-0.75, P = 0.000) and 7.42 (95%CI: 6.01-9.16, P = 0.294), respectively. Subgroup analysis showed that the AUC value of circRNA expression profile in plasma (0.84) was higher than that in tissue (0.78), but there was no statistical difference in Z-test (P = 0.236). Conclusion CircRNA expression profile has relatively high diagnostic value in gastric cancer and it is expected to become an auxiliary diagnostic indicator for gastric cancer.

109 Effect of Yinhuang Liyan Decoction on oral mucositis associated with chemoradiotherapy for nasopharyngeal carcinoma

Objective To explore the clinical effect of Yinhuang Liyan Decoction on chemoradiotherapy related oral mucositis in patients with nasopharyngeal carcinoma. Methods A total of 112 patients received chemoradiotherapy for locally advanced nasopharyngeal carcinoma in Hu′nan Province Hospital of Traditional Chinese Medicine from June 2016 to January 2018 were selected. According to the random number table method, the patients were divided into intervention group and control group, with 56 patients in each group. Both groups were treated with cisplatin chemotherapy and concurrent intensity modulated radiotherapy (IMRT). The two groups received basic oral care during chemotherapy. The intervention group also took Yinhuang Liyan Decoction at the same time. The incidence of oral mucositis was compared between the two groups. Oral assessment scale (OAG) scores at 2 weeks, 4 weeks and the end of chemoradiotherapy; serum superoxide dismutase (SOD), malondialdehyde (MDA) and interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-α) were examined by enzyme-linked immunosorbent assay before and after chemoradiotherapy. Results The conditions of oral mucositis in the intervention group was significantly better than that in the control group (P < 0.05); there was no significant difference in the levels of OAG score, serum SOD, MDA, IL-6 and TNF-alpha between the two groups before chemoradiotherapy (P > 0.05). The OAG score of the two groups was significantly higher than that before chemoradiotherapy after 2 weeks, 4 weeks and at the end of chemoradiotherapy (P < 0.05), the OAG score of the intervention group was significantly lower than that of the control group (P < 0.05). At the end of chemoradiotherapy the levels of SOD, IL-6 and TNF-α in the two groups were lower than those before chemoradiotherapy (all P < 0.05), the level of MDA was higher than that before chemoradiotherapy; while the levels of MDA, IL-6 and TNF-α were lower than control group, the level of SOD in the intervention group was higher than control group (P < 0.05). Conclusion Yinhuang Liyan Decoction has a significant effect on the prevention and treatment of oral mucositis during chemoradiotherapy in patients with nasopharyngeal carcinoma, which is beneficial to reduce the degree of inflammatory reaction caused by chemoradiotherapy and can improve the level of antioxidant index.

Objective To explore the correlation of BCRF-1 mRNA expression with nasopharyngeal carcinoma and its influence on immune function. Methods A total of 54 patients with nasopharyngeal carcinoma (nasopharyngeal carcinoma group) who were admitted to the First Hospital of Changsha (“our hospital” for short) from January 2017 to May 2018 were enrolled in the study. At the same time, 50 cases of non-neoplastic nasopharyngeal patients treated at the same time in our hospital were selected as the control group. The expression of BCRF-1 mRNA was detected by real-time PCR, the T cell subsets were determined by flow cytometry. Results Compared with control group, the relative expression of BCRF-1 mRNA was significantly increased (P < 0.05). And the relative expression of BCRF-1 mRNA in TNM stage T3-T4 and N2-N3 was significantly higher than that in TNM stage T1-T2 and N0-N1 stage (P < 0.05). The percentage of CD4+ T cells and CD4+/CD8+ in the high expression of BCRF-1 mRNA group was significantly lower than that in the low expression of BCRF-1 mRNA group, while the proportion of CD8+ T cells was significantly higher than that in the low expression of BCRF-1 mRNA group (all P < 0.05). Conclusion The expression of BCRF-1 mRNA are may be related to TNM stage T and N of nasopharyngeal carcinoma, and its high expression can inhibit the immune function of patients.

117 The relationship between prognostic nutritional index and clinicopathological characteristics and prognosis of patients with primary hepatocellular carcinoma

Objective To study the relationship between prognostic nutritional index (PNI) and clinicopathological characteristics and prognosis of patients with primary hepatocellular carcinoma. Methods Ninety patients with primary liver cancer admitted to the Second Affiliated Hospital of Xinjiang Medical University from January 2010 to March 2015 were selected for the study. According to the difference of PNI, 60 cases of low PNI group (PNI < 45) and 30 cases of high PNI group (PNI ≥ 45) were divided into two groups. The relationship between PNI and the general data of patients with primary hepatocellular carcinoma was analyzed. In addition, all patients were followed up for 3 years. The relationship between 3-year overall survival rate and clinicopathological features, PNI and treatment modalities were analyzed. Multivariate Logistic regression analysis was used. Results The proportion of low PNI group with age over 60 years and clinical stage Ⅲ-Ⅳ were higher than those of PNI group, and the differences were statistically significant (all P < 0.05). There were no significant differences in gender, smoking, alcohol consumption and lymph node metastasis between two groups (all P > 0.05). Univariate analysis showed that age, clinical stage, lymph node metastasis, PNI and treatment were all related to the 3-year overall survival rate of patients with primary hepatocellular carcinoma (all P < 0.05). Multivariate Logistic regression analysis showed that age (> 60 years old), clinical stage Ⅲ-Ⅳ, lymph node metastasis, PNI < 45 and non-comprehensive treatment were independent risk factors for 3-year overall survival rate of patients with primary liver cancer (all P < 0.05). Conclusion PNI is closely related to the age and clinical stage of patients with primary hepatocellular carcinoma. In addition, there are many risk factors affecting the prognosis of patients with primary hepatocellular carcinoma. Clinically, such factors should be scientifically monitoring or intervened to better improve the prognosis of patients.

121 Correlation analysis between mitochondrial DNA D-LOOP point mutation and long-term chronic hepatitis and hepatocellular carcinoma

Objective To analyze the changes in bases of mitochondrial DNA D-LOOP in various tissues by collecting specimens of hepatitis B virus (HBV)-associated liver cancer patients, and to explore the correlation between HBV-associated liver cancer and mitochondrial DNA D-LOOP. Methods Forty-six patients with chronic hepatitis B-related hepatocellular carcinoma (HCC) who were admitted to Beijing Youan Hospital, Capital Medical University from October 2008 to December 2019 were enrolled in the study. Thirty-one cases of cancerous tissues and corresponding adjacent tissues and peripheral blood, and 7 cases of adjacent tissues and corresponding peripheral blood, 5 cases of cancerous tissues and corresponding peripheral blood, 11 cases of cancer tissues and corresponding adjacent tissues. At the same time, 11 liver tissues from donors of liver transplantation patients and corresponding peripheral blood were selected as controls. The mitochondrial DNA of the specimen was extracted, primers and PCR reactions were designed to obtain the mitochondrial D-LOOP region, and the PCR product was sequenced for one generation. Results The results of one-generation sequencing data showed that long-term chronic HBV hepatitis caused a large number of single-base mutations in mitochondrial DNA D-LOOP region of inflammatory liver tissue and liver cancer, and the difference between liver cancer tissues and liver tissues was statistically significant (P < 0.05). Conclusion Long-term chronic HBV hepatitis can cause a large number of mitochondrial DNA D-LOOP region mutations in hepatitis tissues. Hepatoma tissue is different from normal liver tissue mutation mode, which has implications for the early diagnosis of liver cancer.

125 The relationship between serum levels of VEGF, bFGF and pulmonary hypertension in patients with chronic obstructive pulmonary disease

Objective To investigate the relationship between serum levels of vascular endothelial growth factor (VEGF), basic fibroblast growth factor (bFGF) and pulmonary hypertension (PAH) in patients with chronic obstructive pulmonary disease (COPD). Methods One hundred and forty-three COPD patients admitted to Shanghai Sixth People′s Hospital Affiliated to Shanghai Jiao Tong University (“our hospital” for short) from June 2015 to December 2018 were selected. According to the diagnostic criteria of PAH, the patients were divided into control group A (COPD without PAH) in 61 cases and observation group (COPD with PAH) in 82 cases. In addition, 40 people with normal lung function who were taken physical examination in our hospital for the same period were selected as the control group B. According to the severity of the condition, the patients with COPD were divided into acute stage (69 cases), remission stage (49 cases), stable stage (25 cases). The observation group was divided into mild PAH group (32 cases), moderate PAH group (27 cases) and severe PAH group (23 cases) according to the degree of increase of pulmonary systolic pressure (PASP). Results The levels of serum VEGF, bFGF and PASP in control group A and observation group were significantly higher than those in control group B, while those in observation group were significantly higher than those in control group A, and the differences were statistically significant (P < 0.05). The levels of VEGF and bFGF in COPD patients in acute stage and remission stage were significantly higher than those in stable stage, while the levels in acute stage were significantly higher than those in remission stage, and the differences were statistically significant (P < 0.05). Serum levels of VEGF and bFGF in moderate PAH group and severe PAH group were significantly lower than those in mild PAH group, while those in severe PAH group were significantly lower than those in moderate PAH group, and the differences were statistically significant (P < 0.05). Serum levels of VEGF and bFGF were positively correlated with PASP in COPD patients without PAH (r = 0.346, 0.438, P = 0.011, 0.006), while serum levels of VEGF and bFGF were negatively correlated with PASP in COPD patients with PAH (r = -0.421, -0.339, P = 0.008, 0.012). Conclusion The levels of serum VEGF and bFGF are abnormally expressed in COPD patients, and their expression levels are closely related to the presence of PASP.

129 Clinical application of Recombinant Human Prourokinase combined with Tirofiban in PCI of ASTEMI patients with high thrombus burden

Objective To investigate the clinical application of Recombinant Human Prourokinase (rhPro-UK) combined with Tirofiban in percutaneous coronary intervention (PCI) of patients with high thrombus burden and acute ST-segment elevation myocardial infarction (ASTEMI). Methods A total of 93 patients with high thrombus burden ASTEMI who underwent PCI at the Sanya People′s Hospital from June 2017 to May 2018 were enrolled. The patients were divided into observation group (47 cases) and control group (46 cases) according to the random number table method. The patients in both groups were given conventional PCI treatment. The control group was slowly injected with Tirofiban at the distal end of the coronary artery by aspiration catheter, while the observation group was treated with rhPro-UK on the basis of the control group. Cardiac function indicators, blood flow related indicators, the incidence of major adverse cardiovascular events (MACE) and adverse reactions were compared between the two groups before and after 2 weeks of treatment. Results Compared with before treatment, the left ventricular ejection fraction in the two groups was significantly increased after treatment, while the left ventricular end-diastolic diameter and left ventricular end-systolic diameter were significantly lower, and the differences were statistically significant (P < 0.05). The proportion of thrombolysis in myocardial infarction (TIMI) in blood flow grade 3 and myocardial perfusion grade 3 in the observation group after treatment were significantly higher than those in the control group, while the corrected TIMI blood flow frame count was significantly lower than the control group, and the differences were statistically significant (P < 0.05). There were no significant differences in the incidence of MACE and adverse reactions between the observation group and the control group, and the differences were not statistically significant (P > 0.05). Conclusion rhPro-UK combined with Tirofiban in ASTEMI patients with high thrombus burden who underwent PCI has better curative effect and higher safety of prognosis. It has a good effect on the heart function and blood flow of patients, and it is worthy of clinical promotion.

133 Study on the relationship between blood sugar control level and anti-tuberculosis efficacy in patients with diabetes mellitus complicated with tuberculosis

Objective To explore the relationship between blood sugar control level and anti-tuberculosis efficacy in patients with diabetes mellitus complicated with tuberculosis. Methods Eighty-nine cases of patients with diabetes mellitus complicated with tuberculosis in Wuhan Lung Hospital were selected as study group from January 2013 to September 2018, and another seventy-three cases of patients with single tuberculosis during the same period were selected as control group. The positive rate of sputum bacteria, the rate of cavity, the distribution of pathogenic sites, clinical symptoms and adverse reaction during treatment were observed and compared between two groups. According to the control of fasting blood glucose after treatment, study group was divided into three subgroups: ideal control group (n = 32), good control group (n = 29), and poor control group (n = 28). The therapeutic effects of three subgroups and control group were observed and compared. Results The cavity rate, hemoptysis rate, pulmonary tuberculosis poisoning rate and sputum positive rate of study group before treatment were higher than those of control group, and the differences were statistically significant (P < 0.05). There was no significant difference in lesion location between two groups before treatment (P > 0.05). The lesion absorption rate, sputum negative conversion rate, clinical symptoms improvement rate and cavity shrinkage rate of ideal blood sugar control group, good blood sugar control group were higher than those of poor blood sugar control group, the differences were statistically significant (P < 0.05), and the above indexes of ideal blood sugar control group were higher than those of good blood sugar control group, the differences were statistically significant (P < 0.05). There were no significant differences in above indexes between ideal group and control group (P > 0.05). The above indexs of good blood suger control group and poor suger control group were lower than those of control group, and the differences were statistically significant (P < 0.05). There were no significant differences in above indexes between ideal group and control group (P > 0.05). Conclusion The curative effect of diabetes mellitus patients with tuberculosis is closely related to their blood sugar control level. It is important to strengthen blood sugar control in the treatment of diabetes mellitus patients with tuberculosis, and does not increase the incidence of adverse reactions.

137 Clinical effect of Huatan Jiangqi Huoxue Decoction combined with conventional Western medicine in the treatment of asthma-chronic obstructive pulmonary disease overlap

Objective To explore the clinical effect of Huatan Jiangqi Huoxue Decoction combined with conventional Western medicine in the treatment of asthma-chronic obstructive pulmonary disease overlap (ACO). Methods One hundred and six patients with ACO admitted to Hubei Provincial Hospital of Traditional Chinese Medicine from June 2017 to December 2018 were selected. They were divided into control group (conventional Western medicine treatment) and study group (Huatan Jiangqi Huoxue Decoction combined with conventional Western medicine treatment) by random number table method, with 53 cases in each group. The clinical efficacy, inflammatory factors, pulmonary function indicators,symptom scores changes and adverse reactions of the two groups were compared. Results After 12 weeks of treatment, the total effective rate of the study group was higher than that of the control group, and the difference was statistically significant (P < 0.05). The first second forced expiratory volume (FEV1), peak expiratory flow (PEF), forced vital capacity (FVC), FEV1/FVC, asthma control test (ACT) score after 12 weeks of treatment in both groups were significantly increased compared with those before treatment, while the study group was significantly higher than the control group, and the differences were statistically significant (P < 0.05). The levels of interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), high-sensitivity C-reactive protein (hs-CRP), and COPD assessment test (CAT) in the two groups after 12 weeks of treatment were significantly lower than those before treatment, while the study group was significantly lower than the control group, and the differences were statistically significant (P < 0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Huatan Jiangqi Huoxue Decoction combined with conventional Western medicine in the treatment of ACO can rapidly improve the clinical symptoms of patients with definite curative effect. At the same time, it can effectively improve the level of inflammatory factors and lung function, and it has better safety.

141 Effect comparation of isosmotic dose of 7.5% hypertonic saline and 20% mannitol on intracranial pressure reduction

Objective To compare the effect of isosmotic dose of 7.5% hypertonic saline and 20% mannitol on intracranial pressure (ICP) reduction. Methods The data of patients with intracranial hypertension caused by severe traumatic brain injury admitted to the Second Affiliated Hospital of Shaanxi University of Traditional Chinese Medicine from January 2015 to June 2018 were retrospectively analyzed. Among them, 30 cases met the inclusion criteria were collected and divided into 7.5% hypertonic saline group (15 cases) and 20% mannitol group (15 cases) according to hypertonic therapy. The reduction of ICP, onset time, effective duration, minimum ICP time, cerebral perfusion pressure (CPP), mean arterial pressure (MAP) and central venous pressure (CVP) were compared between two groups. Serum sodium concentration and plasma osmotic pressure were analyzed before treatment and 1 and 3 hours after treatment between two groups. Results After treatment, ICP in 7.5% hypertonic saline group decreased more than that in 20% mannitol group, the onset time and effective duration in 7.5% hypertonic saline group were longer than that in 20% mannitol group, and the minimum ICP time in 7.5% hypertonic saline group was shorter than that in 20% mannitol group, the differences were statistically significant (all P < 0.05). CPP and MAP of 7.5% hypertonic saline group after treatment were higher than those of 20% mannitol group, the differences were statistically significant (P < 0.05). There was no significant difference in CVP between two groups after treatment (P > 0.05). There were statistically significant differences in plasma osmotic pressure and sodium concentration between two groups before treatment and 1 and 3 hours after treatment (all P < 0.05). There were statistically significant differences in plasma osmotic pressure and sodium concentration between two groups 1 and 3 hours after treatment (all P < 0.05). Conclusion ICP reduction effect of 7.5% hypertonic saline is better than that of 20% mannitol.

145 Clinical significance of red blood cell distribution width in benign and malignant hematologic diseases

Objective To explore the difference and clinical significance of red blood cell distribution width (RDW) between benign and malignant hematologic diseases. Methods Clinical data of 150 patients with hematologic diseases in Department of Hematology of Chuiyangliu Hospital Affiliated to Tsinghua University from December 2015 to April 2018 were retrospectively analyzed, and they were divided into benign diseases group (84 cases) and malignant diseases group (66 cases). The RDW values of two groups compared. Results The RDW value of the malignant disease group was lower than that of the benign disease group, and the difference was highly statistically significant (P < 0.01). There were also differences in RDW between different diseases in the malignant diseases group, and the differences were statistically significant (P < 0.05). Conclusion The RDW value of patients with benign and malignant hematologic diseases is significantly different, which is helpful for the diagnosis and differential diagnosis in benign and malignant hematologic diseases and has certain practical value in clinical practice.

148 One case of off-label drug use under the guidance of therapeutic drug monitoring

A 71-year-old man with bipolar disorder had taken various antidepressants intermittently for many years, but the curative effect is not good. On March 21, 2018, the symptoms were slightly relieved and no adverse reactions occurred after the prescriptions of Escitalopram, Mirtazapine and Oxazepam were given in the outpatient clinic of Beijing Anding Hospital, Capital Medical University ("our hospital" for short). On April 11, 2018, the patient was hospitalized for further therapeutic effects. After admission, Lithium Carbonate, Sodium Valproate and Escitalopram were administered. Based on the actual treatment of the patient, and combined with pharmacokinetics and pharmacodynamics, the clinical pharmacist and doctors decided to give off-label dose of Escitalopram under the therapeutic drug monitoring. Escitalopram reached a steady-state valley concentration of 51.5 ng/mL (the concentration range of reference therapeutic was 15-80 ng/mL) after 2 weeks. After that, the patient′s plasma drug concentration remained stable, and the symptoms continued to relieve. Finally the patient achieved clinical improvement and discharged. In this case, based on Therapeutic Drug Monitoring in Neuropsychopharmacology: Update 2017, the clinical pharmacist developed individualized doses for elderly patients with poor conventional dose therapy. Therapeutic drug monitoring in the case of overdosing ensured patient safety as well as efficacy.

152 Clinical effect of Calcium Dobesilate combined with hypoglycemic agents in the treatment of diabetic retinopathy with cataract

Objective To explore the clinical efficacy of Calcium Dobesilate combined with hypoglycemic agents in the treatment of diabetic retinopathy with cataract. Methods A total of 86 diabetic retinopathy patients (118 eyes) with cataract admitted to Yingkou Central Hospital of Liaoning Province from January 2016 to May 2018 were selected and divided into the control group (43 cases, 60 eyes) and the observation group (43 cases, 58 eyes) according to random number table method. The control group was treated with routine hypoglycemic agents, while the observation group was treated with Calcium Dobesilate on the basis of routine hypoglycemic agents. After 3 months of treatment, the clinical efficacy and adverse drug reactions of the two groups were compared. The naked vision of the two groups before and after treatment was observed, and the blood glucose indexes, hemorheology indexes, the levels of vascular endothelial growth factor (VEGF) and hypoxia-inducible factor (HIF-1α) were detected and compared in the two groups. Results The total effective rate of the clinical efficacy of the observation group was higher than that of the control group, and the difference was statistically significant (P < 0.05). After treatment, the naked vision of both groups was increased compared with that before treatment, and the observation group was higher than the control group (P < 0.05). After treatment, the levels of fasting blood glucose (FBG), 2 h postprandial blood glucose (2hPBG) and glycosylated hemoglobin (HbA1c) in the two groups were reduced compared with those before treatment (P < 0.05). After treatment, erythrocyte deformation index, plasma viscosity, erythrocyte aggregation index, hematocritm, serum VEGF and HIF-1α levels in both groups were reduced compared with those before treatment (P < 0.05), and the above indexes in the observation group were lower than those in the control group (P < 0.05). No adverse reactions were found during treatment in the two groups. Conclusion The combination of routine hypoglycemic agents and Calcium Dobesilate in the treatment of diabetic retinopathy with cataract has a significant effect, which can effectively control blood glucose and improve vision, and medication is safety.

156 Effect of health education intervention based on self-efficacy theory on quality of life of patients with gastric cancer

Objective To evaluate the effect of health education intervention based on self-efficacy theory on postoperative quality of life of patients with gastric cancer. Methods Ninety-six patients with gastric cancer who underwent surgical resection in the First Affiliated Hospital of Xi′an Jiaotong University (“our hospital” for short) from May 2016 to August 2017 were numbered according to their registration order of treatment. They were divided into two groups by random number table method. The control group received routine care plan, and the observation group received the health education intervention based on the self-efficacy theory on basis of the control group. The scores of self-rating anxiety scale (SAS), visual analogue scale (VAS), quality of life questionnaire for cancer patients QLQ-C30 and general self-efficacy scale (GSES) were compared between the two groups. Results Before surgery, there were no statistically significant differences in SAS, VAS, QLQ-C30 (physical function, cognitive function, emotional function, social function) and GSES scores between the two groups (all P > 0.05). After surgery, SAS and VAS scores of the two groups were lower than those before surgery, and both SAS and VAS scores of the observation group were lower than those of the control group; in addition, the scores of QLQ-C30 (physical function, cognitive function, emotional function, social function) and GSES of the two groups were higher than the preoperative scores, and the scores of QLQ-C30 (physical function, cognitive function, emotional function, social function) and GSES of the observation group were also significantly higher than those of the control group, the differences were all statistically significant (all P < 0.05). Conclusion Health education intervention based on self-efficacy theory can reduce the pain of patients, relieve the psychological pressure of patients, improve the physical function, cognitive function, emotional function and social function of patients, effectively improve the quality of life of patients, and promote rapid recovery after surgery.

160 Application effect of mission passport manual in outpatient management of asthmatic children

Objective To explore the effect of mission passport manual on self-management and asthma control in children. Methods A total of 32 asthmatic children admitted to the pediatric clinic of the First Affiliated Hospital of Guangzhou Medical University from June to September 2018 were included in the study. Guide the children to complete the self-management task according to the task passport during each outpatient visit, after 3 outpatient visits, the task completion of the children was counted and the results were measured using the self-made inhaling technique score scale and the questionnaire for asthma control in children. Results On average, each child could complete 10.25 tasks. The inhalation technique score, asthma control score and the proportion of patients with asthma control after intervention were higher than before intervention, with highly statistically significant differences (all P < 0.01).  Spearman correlation analysis showed task completion number was positively correlated with asthma control score (r = 0.657, P < 0.01). The linear regression equation of asthma control as the dependent variable obtained two independent variables, task completion quantity and inhalation technique score, which explained 54.8% of asthma control variation, and task completion quantity had a greater effect. Conclusion The application of task passport manual in asthma outpatient management can effectively promote the self-management of children, and has a significant role in improving drug absorption technology and enhancing asthma control.

164 Influence of nursing post management on organizational justice perception of nurses

Objective To explore the role of nursing post management in improving nurses′ sense of organizational justice, so as to provide theoretical reference for other hospitals to carry out post management. Methods Through the establishment and implementation of the post assessment, scientific post setting, manpower allocation, salary distribution, level progression and 360 degree assessment of the post management system. From November 1, 2018 to December 31, 2018, an online questionnaire survey was conducted 510 nurses in a tertiary general hospital using the organizational justice scale. Meanwhile, the turnover rate of nurses before and after nursing post management were collected. Results Totally 510 questionnares were sent out, and 500 valid questionnares were collected, and the recovery rate was 98.04%. The organizational justice scale score of nurses was (36.73±10.32)scores. The overall situation was fair. There was no significant difference in the total score of organizational justice perception and the scores of each item among nurses of different working age, educational background, professional title and labor relations (P > 0.05). The turnover rate of nurses was gradually decreasing (P < 0.01). Conclusion The nursing post management mode is helpful to establish a fairly fair performance distribution system, improve nurses′ sense of fairness and promote the stable development of nurses.

168 Effect of incentive psychological intervention combined with relaxation training in induced labour patients

Objective To approach effect of incentive psychological intervention combined with relaxation training in induced labour patients. Methods Clinical data of 120 cases of induced labour in Suining Center Hospital of Sichuan Province from December 2013 to December 2018 were retrospectively analyzed, they were divided into two groups according to different intervention, control group (routine care intervention and, observation group (incentive psychological intervention cooperate relaxation training), with 60 cases in each group. The scores of SAS and SDS of patients in two groups before and after intervention were observed; the contractions begin time, fetal delivery time, total births time, post-partum bleeding, nursing satisfaction and compatibility of two groups were detected. Results There were no statistically significant difference in scores of SAS or SDS of patients between two groups before intervention (P > 0.05). After intervention, the scores of SAS and SDS of patients in two groups were lower than those before intervention, and the scores in observation group were lower than those in the control group, the differences were statistical significance (P < 0.05). There was no statistically significant difference in the contractions begin time between two groups (P > 0.05). The fetal delivery time and total births time of observation group were shorter than control group, and the post-partum bleeding of observation group was lower than the control group, the differences were statistical significance (P < 0.05). The nursing satisfaction and compatibility of observation group were higher than control group, the differences were statistical significance (P < 0.05). Conclusion The application of induced labour patients by incentive psychological intervention combined with relaxation training can improve anxiety and depression, increase induction efficiency, decrease post-partum hemorrhage, improve nursing satisfaction and compatibility.

Syndrome differentiation is the spleen of cold evil, the spleen Yang depressed, the main acupuncture points were the foot Yangming meridian, Zutaiyin meridian and Renmai point, have a complete. Through the analysis of this midumbilical pain acupuncture effective clinical case, to explore the correlation between the spleen in the five zangs and the navle as the clinical symptom and the acupoint reaction point, share the personal experience of belly button diagnosis and treatment of five viscera diseases with the same way.

186 Application effect of failure mode and effect analysis in transfusion safety management in ICU

Objective To explore the application effect of failure mode and effect analysis in transfusion safety management of critical care medicine. Methods A total of 662 patients admitted to the Department of Intensive Care Unit, Taihe Hospital of Shiyan City from August 2017 to July 2018 and requiring blood transfusion were selected as research objects. Among them, 342 patients from August 2017 to January 2018 served as the control group, 320 patients from February to July 2018 were included in the observation group. The observation group performed blood transfusion according to the traditional nursing routine, while the control group used failure mode and effect analysis to analyze and improve the previous blood transfusion process. The risk coefficients and blood transfusion safety management scores of 7 sub-procedures were compared between the two groups. Results The risk coefficients of the observation group were significantly lower than those of the control group (P < 0.01). The blood transfusion safety management score of the observation group was higher than that of the control group (P < 0.05). Conclusion The transfusion safety management based on the failure mode and effect analysis can effectively reduce the risk in the transfusion process of patients and significantly improve the quality of transfusion safety management, which is worthy of clinical application.
