7 Effect of Fibrauretin on the protein expression and activation of NF-κB in lung tissues of mice with acute lung injury

Objective To study the effect of Fibrauretin on the protein expression and activation of nuclear factor kappa B (NF-κB) in lung tissues of mice with acute lung injury (ALI). Methods Sixty male SPF Kunming mice were randomly divided into 6 groups by drawing lots (10 mice in each group): ALI group [(intratracheal instillation of 5 mg/kg lipopolysaccharide (LPS)], normal control group (sham operation), positive control group (3.3 mg/kg Hydrocortisone Injection + LPS), low, medium and high doses of Fibrauretin groups (2, 10 mg/kg and 50 mg/kg Fibrauretin Injection + LPS). The positive drug and Fibrauretin were given by intraperitoneal injection once a day for 3 consecutive days before establishment of ALI model. Immunohistochemistry was used to detect the expression and localization of NF-κB p65 protein in lung tissues. Western blot was used to detect the expression of phosphorylated NF-κB p65 protein in lung tissues. Results Compared with ALI group, the percentage of positive cells with the entry of NF-κB p65 protein into the nucleus in lung tissues of mice in medium and high doses of Fibrauretin groups was decreased significantly, the difference was statistically significant (P < 0.05). Compared with ALI group, the protein expression of phosphorylated NF-κB p65 in lung tissues of mice in low, medium and high doses of Fibrauretin group was down-regulated significantly, the differences were statistically significant (P < 0.05 or P < 0.01). Conclusion Fibrauretin has preventive effect on LPS-induced acute lung injury in mice, and the effect is related to the inhibition of the protein expression and activation of NF-κB.

11 Study on protective effect of bakuchiol on photoaging HSF cells

Objective To study the protective effect of bakuchiol on photoaging human skin fibroblasts (HSF) induced by ultraviolet radiation B (UVB) irradiation. Methods HSF cells were cultured in vitro and divided into blank group (normal culture, no UVB radiation), UVB model group (UVB radiation), estradiol group and bakuchiol low (1 μmol/L), medium (10 μmol/L) and high dose (100 μmol/L) groups. Cell viability was detected by MTT assay. The protein expressions of type Ⅰ collagen (COL Ⅰ), matrix metalloproteinase-1 (MMP-1) and matrix metalloproteinase-3 (MMP-3) were detected by Western blot. The expression levels of COL Ⅰ and MMP-1 mRNA were detected by reverse transcription polymerase chain reaction (RT-PCR). Results The optimal UVB radiation dose for photoaging HSF cell model was 25 mJ/cm2. Compared with the blank group, there was no significant difference in cell viability and secretion of MMP-1, MMP-3 and COL Ⅰ between the low, medium and high dose groups of bakuchiol (P > 0.05); the cell viability and COL Ⅰ expression level in the UVB model group decreased significantly (P < 0.05), the protein expression levels of MMP-1 and MMP-3 increased significantly (P < 0.05); the mRNA expression levels of MMP-1 and COL Ⅰ decreased significantly (P < 0.05). Compared with the UVB model group, the cell viability and COL Ⅰ expression in the low, middle and high dose groups of bakuchiol increased significantly (P < 0.05), the protein expression levels of MMP-1 and MMP-3 decreased significantly (P < 0.05), the mRNA expression levels of MMP-1 and COL Ⅰ increased significantly (P < 0.05). Conclusion Bakuchiol can promote the expression of COL Ⅰ in HSFs, inhibit the expression of MMP-1 and MMP-3, protect UVB from damage to HSFs and exert anti-photoaging effects on skin.

16 Experiment study of preparation of homomycin nanoparticle targeting probe and its ability to target NPC cells in vitro

Objective To prepare a nano-scale ultrasound molecular probe carrying hypocrellin (H) and targeting folic acid (FA) molecule, and to investigate its basic characteristics and in vitro targeting ability. Methods H and polylactic acid-glycolic acid (PLGA) were dissolved in organic solvents in a certain proportion. Nano-scale ultrasound contrast agent (PLGA/H) containing H was prepared by double emulsification method. The basic characteristics, entrapment efficiency and drug loading of PLGA/H were observed and measured, and the optimal dosage of H was optimized. After the FA molecule was attached to the surface of PLGA by adding folate-conjugated phospholipid DSPE-PEG (2000) Floate, nanoparticles with both H and FA molecular targeting function (PLGA-FA) were prepared by double emulsion method.  PLGA/H and PLGA-FA/H were respectively applied to human nasopharyngeal carcinoma (NPC) cell lines CNE2 (high expression of FA receptor) and CNE2 (low expression of FA receptor) in vitro to observe and compare their targeting. Results The nanoparticle PLGA/H carrying H was successfully prepared. The average particle size of the nanoparticles was (220.50±91.37) nm, the shape was round, the size was uniform and the dispersion was good. PLGA-FA/H nanoparticles can be targeted to CNE2 cells (high expression of FA receptor) of human NPC cell line, while CNE2 cells (low expression of FA receptor) with NPC cell line have no targeted binding. Conclusion PLGA-FA/H, a kind of contrast-enhanced ultrasound nanoparticle with H and FA molecular targeting function, has been successfully prepared, and it can target CNE2 cells (high expression of FA receptor) of human NPC cell line.

20 Intervention effect of Wumoyin Decoction on functional dyspepsia rats of spleen deficiency and Qi stagnation syndrome

Objective To study the effect of Wumoyin Decoction on FD rats of spleen deficiency and Qi stagnation syndrome. Methods Ten rats were randomly selected as normal group from 70 female SD rats, and other rats were modeled by "chronic restraint + swimming + irregular eating" compound factor method. 48 modeled rats were randomly divided into model group, Domperidone group, Chaishao Liujunzi Decoction group, Wumoyin Decoction high, middle and low dose group, 8 rats in each group. Besides normal group and model group, the other groups of rats were given the corresponding drug intervention for 14 days. The food intake and body weight were recorded, the small intestine propulsion rate was determined, the expression of iNOS, SP, 5-HT in antral tissue and hypothalamus were detected. Results Compared with model group, the food intake of rats in normal group and administration groups were increased significantly on the 5th, 10th and 15th day of administration (P < 0.05 or P < 0.01). Compared with model group, body mass of rats in normal group and Domperidone group were increased significantly on the 5th day of administration (P < 0.05 or P < 0.01). Compared with model group, body mass of rats in normal group and administration groups were increased significantly on the 10th and 15th day of administration (P < 0.05 or P < 0.01). Compared with model group, the propulsive rate of small intestine in normal group and administration groups were increased significantly (P < 0.01). The expression of iNOS in gastric antrum tissue of rats in administration groups and normal groups were significantly down-regulated compared with model group (P < 0.05). Compared with model group, SP expression in gastric antrum tissue of Domperidone group and Wumoyin low dose group rats were significantly deed (P < 0.01). Compared with the model group, the expression of 5-HT in gastric antrum tissue of rats in high dose group of Wumoyin was up-regulated (P < 0.05). Compared with model group, the expression of iNOS in hypothalamus of normal group, Domperidone group, Chaishao Liujunzi Decoction group and Wumoyin medium dose group were significantly increased (P < 0.05 or P < 0.01). Compared with model group, the expression of SP in hypothalamus of normal group, Chaishao Liujunzi Decoction group and Wumoyin medium dose group rats was significantly increased (P < 0.05 or P < 0.01). Conclusion Wumoyin Decoction has certain curative effect on FD rats of spleen deficiency and Qi stagnation syndrome, regulating the expression of iNOS, SP and 5-HT in the brain-gut axis may be one of its mechanisms.

25 Effects of allicin on inflammatory mediators, oxidative stress and organ function in rats with severe heat stroke

Objective To observe the effect of allicin on the levels of inflammatory factors, antioxidation and organ function in rats with severe heat stroke. Methods Sixty male SD rats were randomly divided into normal control group (n = 20) and heat shock group (n = 40). Forty rats in heat shock group were randomly divided into saline group and allicin group, with 20 rats in each group. All rats in the heat shock group received adaptive training of environmental simulation for 2 weeks before the start of the experiment. At the last 10 d of adaptive training, the allicin group was given 20 mg/kg of allicin orally at regular intervals every day, and the saline group was given saline with the same proportion of body weight. After adaptive training, rats in the heat shock group were placed in the simulated hot climate animal cabin for 90 minutes. Femoral artery blood was taken from all rats 24 h after cabin exit to detect serum interleukin-17 (IL-17), interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), serum hs-CRP, malondialdehyde (MDA), superoxide dismutase (SOD), total antioxidant capacity (T-AOC), and advanced protein oxidant products (AOPPs). Serum creatinine (SCr), blood urea nitrogen (BUN), alanine aminotransferase (ALT) and aspartate aminotrans ferase (AST) were measured by automatic biochemical analyzer. Results Compared with the normal control group, the levels of IL-17, IL-1β, TNF-α and hs-CRP in serum of rats in allicin group and saline group were significantly increased after heat shock and exercise treatment, the lipid peroxide MDA and AOPPs increased significantly, while the antioxidant SOD and T-AOC decreased significantly, and the levels of liver and kidney biochemical indexes SCr, BUN, ALT and AST were significantly increased, the differences were statistically significant (P < 0.01). However, compared with the saline group, the levels of IL-17, IL-1β, TNF-α, hs-CRP, MDA and AOPPs in serum of rats in allicin group were significantly decreased, the SOD and T-AOC increased significantly, the levels of liver and kidney biochemical indexes SCr, BUN, ALT and AST were significantly decreased, the differences were statistically significant (P < 0.01). Conclusion After heat shock and exercise treatment, the production and release of inflammatory factors and the level of lipid peroxidation in serum of rats increased. Allicin can protect the organs by reducing the production and release of inflammatory factors and lipid peroxidation in the serum of rats with severe heat stroke.

29 Study on the improvement of quality standard of Xinwuyou Tablets

Objective To eastablish and improve the quality standard of Xinwuyou Tablets. Methods Thin-layer chromatography was used to identify Asari Radix et Rhizoma, Artemisiae Scopariae Herba and Salviae Miltiorrhizae Radix et Rhizoma, and high performance liquid chromatography was applied for the determination of salvianolic acid B in Xinwuyou Tablets. Kromasil C18 column (4.6 mm×250 mm, 5 μm) was adopted with a mobile phase of acetonitrile-0.1% phosphoric acid (25∶75) at the flow rate of 1.0 mL/min. The detection wavelength wad set at 286 nm and the column tempetature was 30℃. Results The thin-layer chromatography identification was highly specific. The method of content determination was simple and accurate. The good linearity with peak areas was achieved in the range of 0.0735-3.6750 μg (r = 1.0000) for salvianolic acid B. The average recovery was 101.20% and the RSD was 1.05%. Conclusion The method is simple, accurate and reproducible, and can be used for the quality control of Xinwuyou Tablets.

34 Study on antipyretic mechanism of Qingkailing Soft Capsules based on metabonomics

Objective To evaluate the antipyretic effect of Qingkailing Soft Capsules based on metabonomics technology and to explore its related mechanism. Methods A total of 18 SD male rats were randomly divided into control group, model group and administration group, with 6 rats in each group. Model group and administration group were feverished by dry yeast. Rats in the administration group were given 0.5% CMC-Na suspension of Qingkailing Soft Capsules with baicalin as 12.5 mg/kg and model group and blank group were intragastrically administered with 0.5% CMC-Na at the same volume. The metabolic profiles of rat plasma were analyzed under positive and negative ion modes by ultra-performance liquid chromatography-inear ion trap/orbitrap high resolution mass spectrometry (UPLC-LTQ/Orbitrap-MS) metabolomics technique. Through the multivariate statistical analysis method, the data of each group were compared and analyzed, and the potential biomarkers related to fever in plasma were searched. The reverse regulation of Qingkailing Soft Capsules on these endogenous components was explored, and the possible metabolic pathways were inferred. Results On the principal component analysis (PCA) chart, the plasma metabolite profiles of the control group, model group and administration group were separated obviously. Eight potential plasma biomarkers were found and identified, which were lactic acid, nicotinic acid, 3-furoic acid, succinic acid, phosphocholine, arachidonic acid, LysoPE [0∶0/22∶4 (7Z, 10Z, 13Z, 16Z)] and oleic acid. Compared with the control group, the contents of lactic acid (P < 0.01), nicotinic acid (P < 0.01), 3-furoic acid (P < 0.05), succinic acid (P < 0.01) were increased significantly, while the contents of phosphocholine (P < 0.05), arachidonic acid (P < 0.01), LysoPE [0∶0/22∶4 (7Z, 10Z, 13Z, 16Z)] (P < 0.01), oleic acid (P < 0.05) were decreased significantly, which were in the model group. The contents of eight biomarkers in plasma of rats in the administration group were significantly reversed (P < 0.05 or P < 0.01). Conclusion Metabonomics can be used to evaluate the efficacy of Qingkailing Soft Capsules, which may play an antipyretic role by regulating energy metabolism and phospholipid metabolism.

39 High performance liquid chromatography determination of fucoxanthin content in Sargassum thunbergii of the East China Sea

Objective To establish an high performance liquid chromatography method for the determination of fucoxanthin content in Sargassum thunbergii of the East China Sea. Methods The samples (20 L) were separated on a Sepax Welch C18 column (4.6 mm×250.0 mm, 5 μm) with a gradient elution of ethyl acetate and 90% aqueous methanol at a flow rate of 1.0 mL/min and detected at 450 nm. The column temperature was kept at room temperature. Results A good linearity was obtained in the test range of 2.7-43.2 μg/mL (r = 0.9997). The limit of detection (3S/N) was 0.027 μg/mL and the limit of quantitation (10S/N) was 0.086 μg/mL. The average recoveries of fucoxanthin were between 99.6% and 105.0%, with a RSD of 1.78% (n = 9). Conclusion The method is simple, sensitive, accurate, and can be used for the quality control of fucoxanthin.

43 Research progress on biodegradable non-metallic synthetic bone tissue regeneration materials

The treatment of bone defects is still a major challenge for orthopedics. Large bone defect repair can be accomplished using certain implant materials. The development of new degradable bone regeneration materials, the modification of existing bone repair materials and the application of new preparation processes are the development trends in the field of bone repair materials research. Among the biodegradable non-metallic synthetic materials, bioceramics are the most widely used biodegradable bone repair materials because of their similar composition and relatively high strength. Bioglass can form a tight connection with body tissue and can be used for surface treatment of orthopedic implants to improve implant compatibility. Biopolymers have high biological activity and loose structure, and have great application value in tissue engineering. The formulation of strategies for the application of this kind of materials should include comprehensive plans of materials science, biology science, clinical medicine and other disciplines. The technical means include the development of new formulations, doping of additives, application of new preparation techniques, etc. This article will review the advantages, disadvantages, development and preparation of these types of biodegradable synthetic bone tissue recycled materials.

47 Research progress on clinical application of transcranial magnetic stimulation in epilepsy

Epilepsy is one of the common diseases of the central nervous system, which seriously endangers human health. About 30% of patients with epilepsy can not be effectively controled through the treatment of existing anti-epileptic drugs, so neuromodulation methods such as vagus nerve stimulation, deep brain stimulation and transcranial magnetic stimulation have become the focus of attention. Based on the multiple advantages of non-invasive, painless, effective, and convenient operation, transcranial magnetic stimulation is gradually applied to the diagnosis and treatment of patients with epilepsy. This article reviews the content of transcranial magnetic stimulation in the diagnosis and treatment of epilepsy, summarizes its latest research progress, and points out its shortcomings and defects, and develops an optimized and personalized transcranial magnetic stimulation solution for the diagnosis and treatment of epilepsy in the future.

Stroke is a common and frequently-occurring disease of the nervous system, which is a serious threat to human health. Acupuncture treatment of stroke has been widely recognized, acupuncture can effectively inhibit the cascade reaction and neuronal damage after cerebral ischemia. This article discusses the mechanism of electroacupuncture intervention for cerebral ischemia-reperfusion injury, and provides a theoretical basis for the treatment of this disease by electroacupuncture.

54 Research progress on prevention and treatment of constipation after orthopedic surgery

Constipation is a common complication after orthopedic surgery, its incidence rate can reach to 50%. It causes the patient′s physical and psychological pain, and also prolongs the hospitalization time, increases the medical expenses, and affects the rapid recovery after surgery. This article discusses the causes, diagnosis and prevention of constipation after orthopedic surgery, focusing on the inductive summarization of Western medicine laxatives, chloride channel agonists, prokinetic drugs, peripheral μ-opioid receptor antagonists commonly used in the prevention and treatment of postoperative constipation and abdomen massage, auricular acupoint pressing and acupoint sticking with Chinese medical method, proposes the possibility of traditional Chinese medicine decoction, biofeedback therapy, acupoint embedding and acupuncture applied to this disease, in order to provide ideas for the prevention of constipation after orthopedic surgery.

58 Research progress of iterative model reconstruction in CT angiography

Iterative model reconstruction (IMR) technology is a new type of iterative reconstruction algorithm. Compared with the traditional filtered back projection and hybrid iterative algorithm, it performs better in reducing noise and improving image quality. Application of IMR reconstruction can reduce the radiation dose and ensure the image quality. In this paper, the clinical application progress of IMR technology in CT angiography of various parts is reviewed, and a simple estimation is made to provide reference for related research.

62 Advance in research on the effects of extracellular vesicles on the biological characteristics of tumor

Tumor growth, invasion and metastasis depend on the "cross-talk" between tumor cells and stromal cells in their microenvironment, which mainly involves the secretion of soluble factors of these two kinds of cells, the release of membrane-tomont particles and the direct contact between cells. These vesicles are known as extracellular vesicles (EVSs), and tumor cells secrete more EVSs than normal cells, which separate proteins, DNA, RNA, and other "contents" into the body through both escrt-dependent and escrt-independent pathways. These "contents" facilitate the transmission of information between tumor cells and stromal cells, thus supporting tumor angiogenesis, invasion and metastasis, drug resistance, and malignant characterization of tumor cells.

66 Research progress on risk factors and nursing prevention of acute mastitis during lactation

Acute mastitis is a common complication among postpartum breastfeeding women. It can easily lead to early weaning and induce physical symptoms, including breast swelling, breast pain and even breast abscess. Acute mastitis can be trigged by a variety of risk factors, such as mothers, newborns and breastfeeding pattern etc. The occurrence of acute mastitis can be prevented by proper nursing methods, such as correct breastfeeding frequency and postures, effective breast milk empty, message, hot compress and psychological counseling. As a result, breastfeeding rate may be raised. Additionally, patient education has a little effect for acute mastitis prevention, it needs more high quality researches to support it.

70 Research progress of Chikungunya viruses

Chikungunya virus (CHIKV) is an insect-borne virus transmitted by Aedes aegypti and Aedes albopictus. It has a high infection rate and can cause arthritis symptoms lasting from months to years. In severe cases, it can cause disability and even death. CHIKV has spread worldwide in recent years, infecting millions of people each year. This review introduces CHIKV from the aspects of clinical symptoms, pathological mechanism and diagnostic methods, among which, the pathological mechanism of CHIKV infection is the key part described. Understanding the pathological mechanism of CHIKV infection will be beneficial to finding methods of prevention and treatment to fight against this arbovirus.

74 Enlightenment of microlecture to clinical transfusion teaching in medical laboratory

At present, undergraduates majoring in medical laboratory are mainly employed in laboratory departments, blood transfusion departments and blood stations of hospitals at all levels. For the professional and highly targeted departments such as blood transfusion department and blood station, clinical knowledge of blood transfusion is particularly important, and students′ mastery of it is directly related to their ability to compete for the current position. With the adjustment of the educational system, degree and category of medical laboratory undergraduate by the Ministry of Education, clinical transfusion must rely on students′ self-study after class to strengthen and consolidate the weakened classroom teaching. However, the lack of independent learning ability of Chinese medical laboratory medicine students makes them not optimistic about the learning status of this course. Based on the current situation of teaching of clinical blood transfusion, the feasibility of small class teaching for clinical blood transfusion is analyzed, and the specific implementation process of the potential challenges and countermeasures are discussed, which are based on micro medical test undergraduate students autonomous learning ability in teaching the strategy research, provide a reference for medical colleges of promote students′ autonomous learning ability.

77 Impact of team-based collaborative learning teaching method on nasogastric indwelling catheter success probability and learning interest in nursing students

Objective To investigate the impact of team-based collaborative learning teaching method on nasogastric indwelling catheter success probability and learning interest in nursing students, in order to solve the difficulty, and provide a new method for improving the success rate of nasogastric indwelling catheter and changing teaching pattern in nursing students. Methods A total of 426 nursing students, from 8 classes of 2016 grade in Medical College of Xi′an International University, according to the random draw method were randomly sampling and divided into two groups, the experiment group (213 students), control group (213 students). The control group was given traditional teaching method for the indwelling catheter, the experiment group was given the team-based collaborative learning teaching method, the success rate and learning interest of nursing in two groups were compared. Results The clear success rate, clear failure rate, giving up clear rate in two groups were compared, the differences were statistically significant (P < 0.01). The learning interest of students in experiment group was higher than control group, the difference was statistically significant (P < 0.01). Conclusion Team-based collaborative learning teaching method can effectively improve the success rate of catheter nursing students, stimulate learning interest, it is worthy of promotion and application.

81 Comparative study of working group model teaching and workshop teaching in rehabilitation medicine teaching in higher vocational colleges

Objective To compare the application of TBL and workshop in rehabilitation medicine teaching in higher vocational colleges. Methods From March to June 2018, 120 students from Class 1 of nursing direction and Class 1 of rehabilitation direction in Northern Beijing Vocational Education Institute were selected as the subjects, 61 students from Class 1 of nursing direction given TBL teaching method were as TBL group, 59 students from Class 1 of rehabilitation direction given workshop teaching method were as workshop group. A unified proposition test paper and a rehabilitation operation process assessment form were used to assess the comprehensive performance of the two groups; the theoretical scores and rehabilitation skills of the two groups was compared; the teaching satisfaction questionnaire was used to evaluate the satisfaction of teachers and students; self-made teaching process assessment form was used to evaluated the teaching process by the experts. Results The theory and practice skills assessment scores in the workshop group were higher than the TBL group, the differences were statistically significant (P < 0.05). The teaching satisfaction between teachers and students in two groups were compared, the differences were not statistically significant (P > 0.05), the expert evaluation in two groups were compared, the differences were not statistically significant (P > 0.05). Conclusion Workshop teaching and TBL teaching method have obvious advantages in improving students′ learning interest, efficiency and self-learning ability. It can deepen students′ understanding and memorizing content, improving teachers′ teaching quality and comprehensive quality, enhance teacher-student interaction and team spirit. But the workshop teaching method is slightly superior to the TBL teaching method in all aspects of evaluation. Both methods can be widely used in practical teaching.

85 Exploration and practice of teaching medical immunology for international students

Medical Immunology is a professional foundations for international students majoring in clinical medicine. It is also a bridge course between preclinical medicine and clinical medicine. It is a subject featured with theory and abstract content. Due to the educational background, learning habits, ways of thinking and language problems of international students, the teaching effect is ineffective. Combined with the teaching practice of medical immunology for international students, this paper analyzes the current teaching situation and existing problems. It is suggested that by strengthening the construction of the curriculum system, continuously improving the professional ability and English level of the teaching staff, optimizing the teaching materials, content of selected textbooks and adopting multiple teaching methods and means, the teaching process of international students can be optimized and the teaching quality can be improved constantly.

89 Application of standardized patients in clinical practice teaching of psychiatry

Objective To explore the application of standardized patients (SP) combined with psychiatric examination skills in clinical practice teaching of psychiatry. Methods From March to July 2017, the interns of Psychiatry in the senior clinical medicine major of Shenyang Medical College were randomly selected from two classes and divided into experimental group and control group (traditional group) with 31 students in each group. The experimental group was given a psychiatric examination outline combined with SP practice, and the control group was giren the traditional bedside practice method. To analysis and comparison of two groups of students′ theoretical examination results, students′ evaluation of practice methods and teachers′ evaluation of students′ practice during class. Results The average score of theoretical examination in the experimental group was significantly higher than that in the control group, with statistical significance (P < 0.05). The comparison of the numbers of the two groups in the four scores >90 points, 80-90 points, 70-79 points and <70 points showed high statistical significance (P < 0.01); the results of questionnaire survey showed that the experimental group was superior to the control group in terms of practice atmosphere, actual operation, learning interest, practical ability, practice content, cooperation and coordination ability, independent thinking, and induction and summary ability, with statistical significance (P < 0.05). Conclusion The application of SP combined with psychiatric examination skill training in Psychiatry practice during the break is practical, can improve students′ understanding of theoretical content, exercise students′ communication and collaboration ability, and it has important application value.

93 Effect of Trastuzumab combined with GEM and CAP chemotherapy on serum T cell subsets, TPS levels and quality of life in patients with HER-2 positive advanced breast cancer

Objective To investigate the effect of Trastuzumab combined with Gemcitabine (GEM) and Capecitabine (CAP) in advanced breast cancer patients with positive human epidermal growth factor receptor 2 (HER-2). Methods Eighty patients with HER-2 positive advanced breast cancer admitted to the Affiliated Hospital of Armed Police Logistics College from August 2010 to April 2015 were selected. According to the random number table, 40 cases in control group were treated with GEM and CAP, and 40 cases in observation group were treated with Trastuzumab on the basis of control group. The disease control and the levels of serum tumor markers indicators [tissue polypeptide specific antigen (TPS), cancer-related glycoprotein antigen 153 (CA153)] before and after treatment, serum T cell subsets (serum CD3+, CD4+, CD8+) levels and quality of life dimensions (role function, emotional function, cognitive function, physical function, social function) score changes were observed between two groups, the occurrence of toxic and side effects and survival rate were also counted between two groups. Results The clinical efficacy of observation group was better than that of control group, and the disease control rate of observation group was higher than that of control group (P < 0.05). After 2 months of treatment, the levels of serum TPS and CA153 in two groups were lower than those before treatment, and the decrease in observation group was more obvious than that in control group (P < 0.05). After 2 months of treatment, the levels of serum CD3+ and CD4+ in two groups were lower than those before treatment, but the levels in observation group were higher than those in control group (P < 0.05). There was no significant difference in serum CD8+ level between two groups before and after treatment (P > 0.05). There was no significant difference in the incidence of leukopenia, nausea, vomiting and thrombocytopenia between two groups (P > 0.05). The scores of cognitive function, somatic function, role function, emotional function and social function in two groups were higher than those before treatment, and those in observation group were higher than those in control group (P < 0.05). There was no significant difference in 1, 2 and 3-year survival rates between two groups (P > 0.05). Conclusion Trastuzumab combined with GEM and CAP chemotherapy can reduce serum tumor marker levels in patients with HER-2 positive advanced breast cancer, improve the clinical control rate of the disease, have less damage to the patient′s immune function and have a high prognosis survival rate with fewer side effects. It can improve the quality of life of patients.

98 Outcomes and cost comparison of laparoscopic appendectomy versus open appendectomy

Objective To compare the effects and costs of laparoscopic appendectomy (LA) and open appendectomy (OA) in treating appendicitis, and analyze the reasons for the differences. Methods The data of 495 cases of appendicitis in the Outpatient and Emergency Department of Xuanwu Hospital of Capital Medical University from January 2014 to November 2016 were retrospectively analyzed, according to different treatment methods, patients were divided into LA group (experimental group, n = 441) and OA group (control group, n = 54). Age, sex, anesthesia classification (ASA), operation time, hospitalization time, antibiotic use time and total cost were compared between the two groups. Results The length of stay in the LA group was shorter than the control group, and the difference was highly statistically significant (P < 0.01). The operation time of experimental group was significantly shorter than that of control group, and the difference was highly statistically significant (P < 0.01). The time of antibiotic treatment in experimental group was shorter than that in control group, and the difference was highly statistically significant (P < 0.01). The intraoperative blood loss in experimental group was less than that in control group, the difference was highly statistically significant (P < 0.01). There was no statistically significant difference in total cost between the two groups (P > 0.05). Conclusion LA is a safe and effective surgical method for appendectomy. Compared with OA, LA shortens hospital stay, operation time and antibiotic use time, and reduces blood loss. LA may be the standard treatment for patients with appendicitis and abdominal pain.

102 Meta analysis of endoscopic third ventriculostomy and ventriculoperitoneal shunt for treating hydrocephalus in chirdren

Objective To investigate the clinical effects of the third ventriculostomy (ETV) and ventriculoperitoneal shunt (VPS) in the treatment of hydrocephalus in children. Methods Relevant literatures collected from the establishment of the database to October 2018 were obtained according to inclusion and exclusion criteria through computer retrieval of China national knowledge network, Wanfang data knowledge service platform, VIP database, PubMed, Embase and Cochrane library databases. The Newcastle-Ottawa scale and Jadad scale were used for literature quality evaluation, and Stata 15.0 software was used for Meta analysis. Results There was no statistically significant difference in the effective rate (OR = 1.50, 95%CI: 0.82-2.74, P = 0.189), the incidence of total postoperative complications (OR = 0.73, 95%CI: 0.36-1.51, P = 0.394), the incidence of intracranial hematoma after operation (OR = 1.16, 95%CI: 0.51-2.66, P = 0.726) of ETV and VPS in the treatment of hydrocephalus in children. The incidence of postoperative intracranial infection (OR = 0.26, 95%CI: 0.12-0.57, P = 0.001), the incidence of postoperative shunt obstruction (OR = 0.17, 95%CI: 0.06-0.43, P = 0.000), the rate of reoperation after operation (OR = 0.20, 95%CI: 0.07-0.56, P = 0.002) were statistically significant. Conclusion ETV is superior to VPS in postoperative complications and whether or not to have another operation. More advantages still need to be further observed in clinical studies.

109 Relationship between serum 25-(OH)D3 level and bone mineral density in patients with rheumatoid arthritis

Objective To study the relationship between serum 25-hydroxyvitamin D3[25-(OH)D3] level and bone mineral density in patients with rheumatoid arthritis (RA). Methods A total of 512 cases of patients with RA admitted to Mianyang Central Hospital of Sichuan Province (“our hospital” for short) from February 2016 to February 2018 were selected as the study group. Another 80 healthy people who underwent physical examination in our hospital at the same time were used as a control group. Serum 25-(OH)D3 levels, bone mineral density, and bone mineral density T values were measured in both groups. According to the X-ray staging criteria of American Rheumatism Society, the study group was divided into 128 cases in stage Ⅰ, 162 cases in stage Ⅱ, 133 cases in stage Ⅲ and 89 cases in stage Ⅳ, and compared serum 25-(OH)D3 and bone of patients with different stages of RA. According to the different levels of serum 25-(OH)D3 deficiency, the study group was divided into 130 cases of serum 25-(OH)D3 deficiency, 168 cases of mild deficiency, 127 cases of moderate deficiency and 87 cases of severe deficiency. The bone mineral density and bone mineral density T values were compared, and the correlation between serum 25-(OH)D3 and bone mineral density and bone mineral density T values in RA patients was analyzed. Results Serum 25-(OH)D3, bone mineral density and bone mineral density T values in the study group were lower than those in the control group (all P < 0.01). Serum 25-(OH)D3, bone mineral density and bone mineral density T values in patients with stage Ⅰ-Ⅳ RA showed a gradual downward trend, with highly statistical significance (all P < 0.01). Bone mineral density and bone mineral density T value of RA patients with serum 25-(OH) D3 deficiency, mild deficiency, moderate deficiency and severe deficiency showed a decreasing trend, with highly statistical significance (all P < 0.01). Pearson correlation analysis showed that serum 25-(OH)D3 was positively correlated with bone mineral density and bone mineral density T value in RA patients (r = 0.523, 0.584, all P < 0.05). Conclusion Serum 25-(OH)D3 level and bone mineral density in RA patients are significantly decreased, and there is a significant positive correlation between them. That is, bone mineral density decreases with the increasing lack of serum 25-(OH)D3 level.

113 Changes of plasma TSP-1 and PAF level in children with acute Kawasaki disease and their clinical significance

Objective To study the changes and clinical significance of plasma platelet activating factor (PAF) and thrombospondin-1 (TSP-1) levels in children with acute Kawasaki disease (KD). Methods A total of 107 children with KD who were treated in West China Hospital of Sichuan University ("our hospital" for short) from January 2015 to January 2018 were selected as KD group. They were divided into coronary artery expansion group (n = 43) and non-coronary expansion group (n = 64) according to echocardiographic findings. Thirty cases of indirect inguinal hernia in our hospital in the same period were selected as control group. The levels of TSP-1, PAF and platelets(PLT) in the acute, subacute and recovery phases of the KD group and the control group were detected by enzyme-linked immunosorbent assay (ELISA). The differences among different indicators of each group were compared. Results The levels of plasma PAF, TSP-1 and PLT in KD group with the acute phase were significantly higher than those in control group (all P < 0.05). The plasma levels of PAF and TSP-1 in KD group increased in recovery, subacute and acute stages (all P < 0.05). The plasma PAF level in coronary artery injury group was significantly higher than that in non-coronary injury group (P < 0.05), but there was no significant difference in plasma PAF level between KD group in the subacute and recovery phases and non-coronary injury group (P > 0.05). There was no significant differences in TSP-1 level and PLT count between coronary artery injury group in the acute, subacute, and recovery phase and non-coronary injury group (P > 0.05). There was no correlation between plasma TSP-1 and PAF, PLT in KD group in the acute phase(r = 0.314, 0.051, P > 0.05), but there was a significant positive correlation between PAF and PLT (r = 0.614, P < 0.05). Conclusion The plasma levels of PAF and TSP-1 are significantly increased in the acute phase of KD, and PAF is significantly increased in coronary artery injury. Both PAF and TSP-1 are involved in the occurrence and development of KD, and might be used as markers for early diagnosis of KD.

117 Effects of Dexmedetomidine on postoperative cognitive dysfunction in the elderly patients undergoing PFN in perianesthesia

Objective To study the effects of Dexmedetomidine on postoperative cognitive dysfunction in the elderly patients undergoing PFN in perianesthesia. Methods Sixty elderly scheduled for PFN surgery under spinal anesthesia were enrolled in Nansha Hospital of the First People′s Hospital of Guangzhou City from January to December 2018 were selected as study objects, and they were divided into Dexmedetomidine group (group D, n = 30) and control group (Group C, n = 30) by using random numeral table method. The patients in group D were intravenous infused 0.3 μg/kg Dexmedetomidine within 10 min as premedication 15 minutes before anesthesia, followed by 0.3 μg/(kg·h) infusion until the end of operation. The patients in group C received intravenous infusion of saline. Patient-controlled intravenous analgesia (PCIA) was started for all patients when they were transferred to the ward. The patients were given Sufentanyl 2 μg/(kg·d) as PCIA 100 mL in group C. Group D were given Dexmedetomidine 1 μg/(kg·d) + Sufentanyl 2 μg/(kg·d) as PCIA 100 mL. The levels of MDA and 8-iso PGF2α in serum were detected before anesthesia (T0), 24 h (T1) and 48 h (T2) after operation. The min-mental state examination (MMSE) scores were estimated at T0, T1 and T2. Results The levels of MDA and 8-iso PGF2α at T1 and T2 in two groups were higher than those at T0, but those in group D were lower than those in group C at the same time, the differences were statistically significant (P < 0.05). MMSE scores of T1 and T2 in two groups were lower than those of T0, but those in group D were higher than those in group C at the same time, the differences were statistically significant (P < 0.05). VAS of T1 and T2 in group C was higher than that in group D, and the times of pressing, effective pressing and the dosage of Sufentanil in group C were higher than those in group D, the differences were statistically significant (P < 0.05). Conclusion Dexmedetomidine can reduce the incidence of postoperative cognitive dysfunction in the elderly patients undergoing PFN in perianesthesia.

121 Effects of two kinds of intraspinal block anesthesia on body temperature during different periods of labor in patients with latent labor analgesia

Objective To compare the effects of two kinds of intraspinal block anesthesia on body temperature during different periods of labor in patients with latent labor analgesia. Methods From June 2017 to July 2018, 240 eligible pregnant who was the first-time mothers in Heibei Petroleum Center Hospital were selected, according to the random number table method, they were randomly divided into continuous epidural block and analgesia group (group A), combined lumbar stiffness block and analgesia group (group B) and no labor analgesia group (group C), 80 cases in each group. Patients in group A were given continuous epidural block anesthesia when the uterine opening was 1-2 cm, and 0.1% Ropivacaine Hydrochloride was given 8 mL. After 30 min, the epidural catheter was connected with the automatic electronic analgesic pump (the electronic pump was equipped with 0.1% Ropivacaine Hydrochloride and 0.5 μg/mL mixed solution of Sufentanil 100 mL). Patients in group B were treated with lumbar hard combined block anesthesia when the uterine opening was 1-2 cm, followed by subarachnoid injection of 0.1% Ropivacaine 3 mg. After 30 minutes, the epidural catheter was connected with the automatic electronic pump. Group C was in normal delivery. Withdrawing the labor analgesia when the delivery was ending. The maternal temperature was measured at the time of delivery, the temperature at 2 cm of the uterine opening, the temperature at 4 cm of the uterine opening, the temperature at the full time of the uterine opening, and the temperature at 2 h and 24 h after the delivery in three groups. All parturients were tested for CRP and WBC by venous blood sampling before and after 24 h. Results The fever rate of >37.3℃ in group A and group B was significantly higher than that in group C, and the difference was statistically significant (P < 0.05). Postpartum CRP levels of three groups were higher than those of prenatal women, and the levels of postpartum CRP in group A and group B were higher than those in group C, the differences were statistically significant (P < 0.05). Conclusion Both kinds of intraspinal anesthesia can cause the rise of body temperature during childbirth, but it can return to normal 24 h after delivery. The increase of body temperature has little relationship with infection.

124 The clustering analysis of intensity-associated parameters of acupressure based on literature research

Objective To study and analyze the clinical application rules of intensity-associated parameters of acupressure. Methods Taking “acupressure” as title index word, the published SCI literatures were retrieved in PubMed database from January 1, 2014 to January 31, 2018. According to the relevant parameter information of 5 types of acupressure, such as magnitude of force, direction of force, point of force application, area of thrust surface, time/frequency, the literatures were divided into “description” and “no description”, and the literatures with “description” were taken descriptive classification and clustering analysis. Results Fifty-five usable data elements were obtained and classified according to the relevant parameters of acupressure, among which, there were 12 data elements that all 5 parameters were described, accounting for 21.8%; 0 data element had “no description”. Conclusion In recent five years, among the acupressure literatures recorded by SCI, the application of intensity-associated parameters is not uniform. There are some problems, such as missing, error and blurry parameters, lower standardization levels of intensity-associated parameters of acupressure

This paper sums up professor Lai Xinsheng′s clinical experience in the treatment of eczema by acupuncture and medicine based on “tongyuan” therapy. Professor Lai Xinsheng thinks that the nature of eczema is deficiency. The deficiency may be caused by disharmony between nutrient qi and defensive qi, which is due to the weakness of the middle triangle, or cased by blood sea deficiency and onset of endogenous wind, or caused by the deficiency of the viscera and meridians. Professor Lai Xinsheng′s “tongyuan” therapy uses acupuncture and medicine to nourish primordial spirit by simulating the governor channel and nourishing the primordial qi. It defends the primordial qi and primordial spirit when driving the evil away, so where there the evil gets away, there will be peaceful.

133 Summary on Lingnan famous doctor Zhen Mengchu′s clinical experience on chronic pulmonary diseases treatment based on the theory of stasis

Zhen Mengchu was one of the first batch of prestigious Chinese physicians in Guangdong Province since the founding of the People′s Republic of China. He believed that the theory of “chronic diseases causing stasis” and “chronic diseases transforming to collaterals” of Ye Tianshi is the theoretical basis of chronic lung disease causing stasis, on the basis of which, he put forward the viewpoint of “chronic lung diseases causing stasis, removing stasis for eliminating syndrome”, elaborated the TCM pathology and pathogenesis of “blood stasis” in chronic pulmonary diseases, he usually used Tiepo Decoction of folk prescription and has achieved good effects. This paper summarizes the experience of Zhen Mengchu in the treatment of chronic lung disease from the theory of stasis, with a classic case, which provides a certain basis for the treatment of chronic lung disease from “blood stasis”.

136 Comparison of readout segmented EPI and single-shot EPI diffusion weighted imaging in patients with breast cancer

Objective To investigate the image quality different of readout segmented echo planar imaging (Rs-EPI) and single-shot echo planar imaging (Ss-EPI) diffusion weighted imaging (DWI) in patients with breast cancer. Methods Thirty-six patients with breast cancer performed MR scan in Beijing Friendship Hospital, Capital Medical University from March to August 2018 were retrospectively enrolled in this study. In all patients, Rs-EPI-DWI and Ss-EPI-DWI were performed. Two independent readers visually assessed lesion edge sharpness evaluation (ESE) and geometric distortion evaluation (GDE). Differences between two sequences were compared quantitatively by measuring the signal-to-noise ratio (SNR), contrast, contrast-to-noise ratio (CNR), and relative apparent diffusion coefficient (rADC). Results ESE, GDE, CNR, and rADC values showed a significant difference between Rs-EPI-DWI and Ss-EPI-DWI (P < 0.01). There was no statistically significant difference in SNR and contrast values between the two sequences (P > 0.05). Conclusion In breast cancer, Rs-EPI-DWI provids significantly higher image quality than Ss-EPI-DWI.

140 Clinical effect of Epalrestat in the treatment of diabetic peripheral neuropathy

Objective To observe the effects of Epalrestat in the treatment of diabetic peripheral neuropathy. Methods One hundred and twenty-three cases of patients with diabetic peripheral neuropathy admitted to Shunyi Hospital, Beijing Hospital of Traditional Chinese Medicine from February 2014 to December 2016 were selected and divided into control group, observation group and combined group by random table method, with 41 cases in each group. The control group was given Mecobalamin Tablets, the observation group was given Epalrestat Capsules, while the combined group was given Mecobalamin Tablets combined with Epalrestat Capsules. All three groups were treated for 12 weeks. Michigan neurological screening scale (MNSI) was used to evaluate the symptoms and signs of neuropathy. The electromyography was used to detect the nerve conduction velocity of exercise and sensory nerves in median nerve, nervus peroneus communis, ulnar nerve before and after treatment. The conditions of adverse reactions were recorded. Results After treatment, the scores of symptoms and signs of neuropathy of MNSI in the three groups were all lower than those before treatment (P < 0.05), which of observation group and combined group were all lower than those of control group (P < 0.05), while there were no statistically significant difference between observation group and combined group (P > 0.05). After treatment, the nerve conduction velocity of exercise and sensory nerves in median nerve, nervus peroneus communis, ulnar nerve in the three groups were all faster than those before treatment (P < 0.05), which of observation group and combined group were all faster than those of control group (P < 0.05), while there were no statistically significant differences between observation group and combined group (P > 0.05). There were no serious adverse reactions in the three groups. Conclusion Compared with Mecobalamin, Epalrestat can improve the symptoms, signs and nerve conduction velocity more effectively, and good safety, but combined treatments of the two drugs don′t show better efficacy.

144 Curative effect of Duhuo Granules combined with Celecoxib on knee osteoarthritis and its influence on related cytokines

Objective To investigate the curative effect of Duhuo Granules combined with Celecoxib on knee osteoarthritis (KOA) and its effect on related cytokines. Methods A total of 101 patients with KOA admitted to Shuyang Hospital Affiliated to Nanjing University of Traditional Chinese Medicine from January 2016 to August 2017 were selected, the patients were divided into control group (n = 50, Celecoxib) and study group (n = 51, Celecoxib combined with Duhuo Granules) according to random digital table method. The clinical efficacy, hypersensitive C-reactive protein (hs-CRP), interleukin-1 (IL-1), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), transforming growth factor-β (TGF-β), clinical symptom score, visual analogue score (VAS), knee joint function (HSS score) and adverse reactions were compared between two groups. Results After 30 d of treatment, the total effective rate of study group was significantly higher than that of control group (P < 0.05). After 30 d of treatment, the levels of hs-CRP, IL-1, IL-6 and TNF-α in two groups were lower than those before treatment, and the levels in study group were lower than those in control group (P < 0.05). After 30 d of treatment, the level of TGF-β in study group was higher than that before treatment, and the level of TGF-β in study group was higher than that in control group (P < 0.05). After 30 d of treatment, the scores of clinical symptoms and VAS in two groups were lower than those before treatment, and those in study group were lower than those in control group (P < 0.05). After 30 d of treatment, HSS score of two groups was higher than before treatment, and that of study group was higher than that of control group (P < 0.05). There was no significant difference in the incidence of adverse reactions between two groups (P > 0.05). Conclusion Duhuo Granules combined with Celecoxib in the treatment of KOA has definite curative effect. It can effectively improve the level of related cytokines, improve the clinical symptoms of patients without increasing the incidence of adverse reactions.

148 Efficacy of Amoxicillin in different dosage regimen on Helicobacter pylori eradication

Objective To observe the clinical efficacy of Amoxicillin in different dosage regimen on Helicobacter pylori (Hp) eradication. Methods From April 2016 to January 2017, 210 cases of Hp infection in the outpatient department of Liuzhou People′s Hospital of Guangxi Zhuang Autonomous Region were divided into two dosing group, three dosing group and four dosing group in accordance with the random number table with 70 cases in each group. Each group was given Raberazole, Clasamycin, Colloidal bismuth pectin, and Amoxicillin 1 g was given twice-daily to two dosing group, Amoxicillin 500 mg was given three times-daily to three dosing group and four times-daily to four dosing group. Hp was examined at 4 weeks after completing treatment. The eradication rate of Hp, the incidence of adverse drug reactions and the compliance of each group were compared. Results The eradication rate of Hp in two-dose group and three-dose group was significantly lower than that in four-dose group, and the difference was statistically significant (P < 0.05). There was no significant difference in the incidence of adverse reactions and medication compliance among groups (P > 0.05). Conclusion Amoxicillin four times daily dosing can significantly improve the eradication rates of Hp.

152 Clinical distribution and drug resistance of Streptococcus agalactiae from 2013 to 2017 in the Second Affiliated Hospital of Hainan Medical University

Objective To investigate the clinical distribution and drug resistance of Streptococcus agalactiae, and to provide evidence for preventing clinical infection and guiding clinical rational drug use. Methods All kinds of clinical specimens were collected from outpatient and inpatient patients of the Second Affiliated Hospital of Hainan Medical College from January 2013 to December 2017. WHONET 5.6 software was used to statistically analyze the types of infection and drug resistance caused by Streptococcus agalactiae, and to conduct the D-Bacteriostatic circle test for Streptococcus agalactiae with Erythromycin resistance, Clindamycin sensitivity or intermediate. Results A total of 261 strains of Streptococcus agalactiae were detected, and the detection rate was increasing year by year. Mainly distributed in obstetrics and gynecology, outpatient and urology. Mainly from vaginal secretions, followed by cleaning midstream urine and wound secretions. No resistant strains to Penicillin, Ampicillin, Vancomycin and Linezolid were found. The drug resistance rates of Cefotaxime, Cefepime and Ceftriaxone were low. And Clindamycin, Erythromycin, Azithromycin, Tetracycline resistance rate were high. Conclusion Streptococcus agalactiae infections are most common in urinary and reproductive system infections and soft tissue infections, and Penicillin and other β-lactamides can be used as the first choice of empiric agents for Streptococcus agalactiae infections. In addition, the addition of D-circle test can effectively avoid the unreasonable use of Clindamycin, and according to the drug sensitivity test results, the rational selection of antimicrobial agents for prevention and treatment.

156 Diagnostic value of serum oxaphosphatase 1 level in α-fetoprotein negative primary liver cancer

Objective To evaluate the diagnostic value and the threshold value of paraoxonase 1 (PON1) in α-fetoprotein negative primary liver cancer (AFP-PHC). Methods From January to December 2015, serum samples of 60 AFP negative cirrhosis patients (AFP-LC group) and 46 AFP-PHC patients (AFP-PHC group) hospitalized in the Affiliated Cancer Hospital of Guangxi Medical University were collected. The serum AFP levels of all included subjects were detected by electrochemoluminescence immunoassay, and PON1 levels were detected by enzyme-linked immunoassay. Results The serum PON1 level in the AFP-PHC group was significantly lower than that in the AFP-LC group, and the difference was statistically significant (P < 0.05), the maximum area under the subject operating characteristic curve for diagnosis of AFP-PHC by PON1 level was 0.768. When PON1 level < 234.31 pg/mL, the sensitivity and specificity of the diagnosis of AFP-PHC were 71.7% and 73.9% respectively. There was no correlation between serum PON1 level and liver function grade, pathological differentiation degree, tumor thrombus or positive hepatitis B surface antigen in patients with AFP-PHC (P > 0.05). Conclusion Serum PON1 level is effective is diagnosing AFP-PHC.

Nation Health Commission of the People′s Republic of China and State Administration of Traditional Chinese Medicine of the People′s Republic of China Issued Guilding Opinions on Standardizing the Administration of Contractual Service from Family Doctors ("the Guilding" for short) On September 29, 2018. The Guideline standardizes the management of family doctor services in terms of the agreement on the subject and object of providing contracted services and further protects the rights and interests of contracted families. Compared with the recipients of the services, the Guideline is not enough to protect the rights and interests of the providers′ family doctors. There are risks in terms of responsibility for service content, service location, payment source training system and other aspects. Therefore, while preventing risks faced by contracted families, risks faced by family doctors of service providers should also be taken into account and prevention.

164 Effect of negative emotion and psychological capital on personal achievement among nurses in a cancer hospital

Objective To explore the relationship model between negative emotions, psychological capital and personal accomplishment in cancer hospitals. Methods In May 2018, 207 nurses from Zhejiang Cancer Hospital were selected by convenient sampling method. The survey was conducted using a general data questionnaire, positive and negative affect scale, Maslach Burnout Inventory, and psychological capital questionnaire, and multiple linear regression and mediating effects were used for analysis. Results The scores of negative emotion, psychological capital and personal accomplishment of nurses in the tumor hospital were (22.00±5.66), (100.50±14.44), (30.43±10.93) scores, respectively. Negative emotions and personal accomplish-ment were negatively correlated (r = -0.322, P < 0.05). The total scores of psychological capital and the scores of each subscale were positively correlated with the scores of individual accomplishments (r  > 0, P < 0.05). The negative emotions and the total scores of psychological capital and the scores of subscales were negatively correlated (r < 0, P < 0.05). Not significant for the direct effect of negative emotion (P > 0.05), the mediating effect of psychological capital (- 0.231) was about four times (95%CI: -0.341, -0.142) that of the direct effect of negative emotion (-0.060). Conclusion Negative emotions can negatively predict individual achievement scores, and mental capital can positively predict individual achievement scores. Negative emotions affect individual achievement scores through the complete mediating effect of psychological capital. Cancer hospital nurse managers should take a variety of measures to help nurses overcome and mitigate the effects of negative and negative emotions. Focus on developing and cultivating the psychological capital of nurses, actively creating a benign work and learning environment, and enhancing their personal accomplishment and professional values.

169 Evaluation of application effect of health education guided by management by objective on transitional care in patients with new-onset type 2 diabetes mellitus

Objective To investigate the application effect of health education directed by management by objective (MBO) on transitional care in patients with new-onset type 2 diabetes mellitus (T2DM). Methods From November 2016 to April 2017, 200 new-onset T2DM patients coded by the order of registration at outpatient diabetes education guided by diabetes specialist nurse of endocrinology at a tertiary hospital were enrolled, according to random number generator algorithm, they were randomly divided into control group(100 cases) and intervention group (100 cases). In the intervention group, patients were under transitional care model directed by MBO, whereas patients in the control group received the routine health management. Glycosylated hemoglobin (HbA1c), body mass index (BMI) and indices of self-management were assessed at baseline and after intervention for 48 weeks in all participants. Results After intervention, HbA1c, BMI in intervention group were better than control group, the differences were statistically significant (P < 0.01). The scores of all items of diabetes mellitus self-management were higher than control group, the differences were statistically significant (P < 0.05). Conclusion As an effective method of transitional care in new-onset diabetic patients, the health education guided by MBO could assist patients to improve self-management and control HbA1c and body weight.

173 Investigation on the status of self-perceived burden and analysis of influencing factors in elderly patients with coronary heart disease

Objective To investigate the status of self-perceived burden and the related influencing factors in elderly patients with coronary heart disease. Methods A total of 229 elderly patients with coronary heart disease from February to September 2018 treated in the First Affiliated Hospital of Anhui Medical University and the Second Affiliated Hospital of Anhui Medical University were collected via convenience sampling method and questionnaire investigated. The general survey questionnaire, self-perceived burden scale (SPBS), medical coping modes questionnaire (MCMQ), hospital anxiety and depression scale (HADS), and general self-efficacy scale (GSES) were adopted to investigate the participants. Multiple linear regression was used to explore the influencing factors of self-perceived burden in elderly patients with coronary heart disease. Results The total score of self-perceived burden was at a mild level of (28.83±7.88) scores. The influencing factors for self-perceived burden included avoidance coping style, anxiety, depression, and self-efficacy (adjusted R2=0.452, P < 0.05). Conclusion The self-perceived burden of elderly coronary heart disease patients is at a mild level, and targeted interventions should be taken to reduce the self-perceived burden and promote their mental health.

177 Application of congenital giant colon surgery in peritoneoscope by perioperative integrated insulation mode

Objective To approach result of congenital giant colon surgery in peritoneoscope by perioperative integrated insulation mode. Methods The clinical data of 140 congenital giant colon surgery children in Xinhua Hospital Affiliated to Xinhua Hospital Affiliated to Shanghai Jiaotong University School of Medicine from October 2016 to October 2018 were analyzed, which was to be divided into two groups by random paper method, control group and detection group, with 70 cases in each group. The control group was given routine thermal insulation nursing measures, and the observation group was given perioperative integrated thermal insulation mode on the basis of the control group. The central body temperature, oxygen saturation (SpO2), bispectralindex (BIS), end-tidal carbon dioxide partial pressure (PetCO2), heart rate (HR) of children in two groups were detected at the time of entering the operating room (T1), 30 min (T2) after anesthesia, 60 min (T3), 90 min (T4), and end of operation (T5), the untoward reaction rate of children in two groups were detected, the family cooperation and satisfaction of children in two groups were compared. Results There was no statistical significance in the comparison of the central body temperature of children in detection group and BIS in control group, SpO2, HR or PetCO2 of children in two groups, the comparison of SpO2, PetCO2 of children between two groups at T1-T5 (P > 0.05); the BIS of children in detection group at T4, T5 were higher than those at T2, T3, and the control group at the same time point, the differences were statistically significant (P < 0.05). There was no statistically significant difference in central body temperature, BIS or HR at T1-T3 between two groups (P > 0.05); the central body temperature of children in control group lower at T4, and T5 was the minimize, the central body temperature of children in detection group at T4 and T5 time point were higher than control group, and the HR was lower than the control group, the differences were statistical significance (P < 0.05). The incidence of recovery time extended, shiver, local pressure injury, hypoxemia rate of detection group children was lower than control group, the family cooperation and satisfaction of children in detection group were higher than control group, the differences were statistical significance (P < 0.05). Conclusion The application of congenital giant colon surgery in peritoneoscope by perioperative integrated insulation mode, which could decrease hypothermia rate, cut down wake up time, decrease complications rate, it is worth to be used.

181 Application effect of humanized intervention management combined with catering posture guidance in cerebral apoplexy patients

Objective To approach effect of humanized intervention management combined with catering posture guidance in cerebral apoplexy patients. Methods From January 2015 to January 2018, in Dalian Municipal Central Hospital, the clinical data of 200 patients with cerebral apoplexy were analyzed, they were divided into general intervention group (100 cases) and joint intervention group (100 cases) by different intervention. The National Institute of Health Stroke scale (NIHSS) score, modified Rankin scale (mRS) score, Barthel index and Fugl-Meyer score of patients in two groups before and after intervention were detected, the Sumida drinking water test grade of patients in two groups were detected. Result The NIHSS score, mRS score, Barthel index and Fugl-Meyer score of patients in two groups before intervention were compared, the differences were no statistical significance (P > 0.05). The NIHSS scores, Barthel index and Fugl-Meyer scores in two groups were higher than before intervention, the mRS scores were lower than before intervention, the differences were statistically significant (P < 0.05); and the NIHSS scores, Barthel index and Fugl-Meyer scores of joint intervention group after intervention were higher than general intervention group, the mRS score was lower than general intervention group, the Sumida drinking water test grade in joint intervention group were better than general intervention group, the differences were statistically significant (P < 0.05). Conclusion The application of cerebral apoplexy patients by humanized intervention management combined with catering posture guidance, which could improve nerve function, Barthel and Fugl-Meyer, increase swallowing function, it is worth to be used.
