| Evaluation of application effect of health education guided by management by objective on transitional care in patients with new-onset type 2 diabetes mellitus |

Abstract Objective To investigate the application effect of health education directed by management by objective (MBO) on transitional care in patients with new-onset type 2 diabetes mellitus (T2DM). Methods From November 2016 to April 2017, 200 new-onset T2DM patients coded by the order of registration at outpatient diabetes education guided by diabetes specialist nurse of endocrinology at a tertiary hospital were enrolled, according to random number generator algorithm, they were randomly divided into control group(100 cases) and intervention group (100 cases). In the intervention group, patients were under transitional care model directed by MBO, whereas patients in the control group received the routine health management. Glycosylated hemoglobin (HbA1c), body mass index (BMI) and indices of self-management were assessed at baseline and after intervention for 48 weeks in all participants. Results After intervention, HbA1c, BMI in intervention group were better than control group, the differences were statistically significant (P < 0.01). The scores of all items of diabetes mellitus self-management were higher than control group, the differences were statistically significant (P < 0.05). Conclusion As an effective method of transitional care in new-onset diabetic patients, the health education guided by MBO could assist patients to improve self-management and control HbA1c and body weight.
