Study of reversal multidrug resistance effect of carboxymethyl chitosan on osteosarcoma MG-63/ADM cells in vitro

Objective To observe the effect of reversing drug resistance of carboxymethyl chitosan (CMC) on human osteosarcoma resistant cell line MG-63/ADM and preliminary explore its mechanism. Methods Human osteosarcoma drug resistant MG-63/ADM cell line was cultured in vitro. The drug resistance was detected by MTT; after 24, 48 and 72 h of osteosarcoma MG-63 /ADM cells were treated with CMC + adriamycin 2.0 μg/mL, and the inhibition rate of cell proliferation in each group was measured by MTT method. The expression of P-gp in MG-63/ADM cells and the expression of NF-κB P65 protein in nucleus of MG-63/ADM cells after treated with the different concentrations of CMC were detected by Western blot. Results MTT results showed that MG-63/ADM cells were resistant to adriamycin, IC50 was (1.97±0.56) g/mL, the resistance index was 11.35; CMC could significantly inhibit MG-63/ADM cells proliferation, and the inhibitory effect was time and dose dependent (P < 0.01). Western blot test results showed that the expression of P-gp in MG-63/ADM group was significantly higher than that in MG-63 group (P < 0.05), and the expression of P-gp in MG-63/ADM treated with different concentrations of CMC was significantly lower than that in MG-63/ADM group (without drugs) (P < 0.05). The expression level of NF-κB/P65 in MG-63/ADM group was significantly higher than that in MG-63 cell group (P < 0.05). The expression of NF-κB P65 in the nucleus of CMC + ADM group was significantly lower than that of the control group (MG-63/ADM) (P < 0.05). Conclusion Carboxymethyl can partly reverse the multidrug resistance in MG-63/ADM cells, and its mechanism may be related to the inhibition of NF-κB signaling pathway and the inhibition of P-gp expression.

The protective effect of baicalin on the damaged SH-SY5Y cells induced by H2O2 through antioxygenation

Objective To investigate the protective effects of baicalin on damaged SH-SY5Y cells induced by H2O2 through antioxygenation. Methods According to the different culture methods, SH-SY5Y cells were divided into five groups: control group (normal culture media), model group (200 μmol/L H2O2), low, medium, high dose of baicalin groups (protected by 50, 100, 200 μmol/L baicalin first, then damaged by H2O2). The survival rate of the SH-SY5Y cells were determined by MTT. The expression of thioredoxin protein (Trx) in cells was detected by Western blot. The contents of superoxide dismutase (SOD), glutathione peroxidase (GSH-Px) and lipid oxidative malonaldehyde (MDA) were detected by kits. Results The absorbancy and cell survival rate in the model group were significantly lower than those of control group (P < 0.01). Compared with model group, the absorbancy and cell survival rate of different doses of baicalin groups were all increased to different degrees, among which, there were highly statistically significant differences of low, medium doses of baicalin groups cpmpared with model group (P < 0.01). Compared with control group, the MDA of model group increased significantly, while the Trx, SOD and GSH-Px decreased significantly (P < 0.01). Compared with model group, the MDA of low, medium doses of baicalin groups decreased, while the Trx, SOD and GSH-Px increased, there were statistically significant differences (P < 0.05 or P < 0.01). Conclusion Baicalin has protective effects on the damaged SH-SY5Y cells induced by H2O2, the mechanism may be related to antioxidation.

Relationship between sperm tail morphology and sperm motility parameters

Objective To analyze the relationship between sperm tail morphology and sperm motility parameters. Methods A total of 15 398 specimens were collected from Reproductive and Genetic Center in Suzhou Municipal Hospital from November 2015 to December 2017. The samples were divided into asthenospermia group (4941 cases) and normal spermatozoa activity group (10 457 cases). The morphological malformation statistics of the two groups were collected. The relationship of sperm tail morphology and sperm motility parameters were analyzed. Results The abnormal morphology rates of asthenospermia group and normal group were significantly different (P < 0.05). There was statistically difference between asthenospermia group and the normal in mid piece angular bend [(9.41±3.62)% vs. (8.33±3.12)%], short [(1.34±2.87)% vs. (0.74±1.08)%], hairpin-like bend [(0.69±0.92)% vs. (0.64±0.81)%], curled [(8.83±4.09)% vs. (7.53±3.52)%], and main piece angular bend [(8.24±2.93)% vs. (7.65±2.58)%] (P < 0.05). The most common types of abnormal sperm tail morphology were mid piece angular bend, curling and main piece angular bend. Sperm motility gradually decreased as the proportion of the three kinds of malformations gradually increased. With the increase of the proportion of these three deformities sperm motility gradually malcreased, the differences were highly statistically significant (P < 0.01). Conclusion Sperm tail morphology malformation on sperm motility parameters has statistically influence. It is necessary to observe the sperm tail morphology of asthenospermia patients.

Effects of Lobeline on chronic neuropathic pain

Objective To investigate the effects of Lobeline on chronic neuropathic pain. Methods Forty-eight mice aged 2 months were divided into 4 groups by random number table method and fed routinely: one group was blank control group, and chronic constriction injury model of sciatic nerve were prepared in the other three groups; model group (CCI + intraperitoneal injection of saline), low dose Lobeline group (CCI + intraperitoneal injection of 3 mg/kg Lobeline solution), high dose Lobeline group (CCI + intraperitoneal injection of 6 mg/kg Lobeline solution). The pain threshold and activity level of mice were measured by Von Fery mechanical pain measurement and open field test on the 1st, 8th, and 22nd days after injection. Results In the determination of pain threshold, the model group decreased on the 1st, 8th, and 22nd days after injection compared with the blank control group (P < 0.05). While compared with the model group, the high dose Lobeline group was elevated in the three experiments (P < 0.05). In the open field experiment, compared with the blank control group, the total distance and average speed of the model group significantly decreased on the 1st and 22nd days (P < 0.01). Compared with the model group, the total distance and mean velocity of the high dose Lobeline group on the 22th day increased significantly (P < 0.01), and the number of activities was improved (P < 0.05). Conclusion The 6 mg/kg Lobeline solution can increase the threshold of mice with chronic neuropathic pain and improve the mice′s depression-like behavior.

Research progress of mild traumatic brain injury biomarkers

The incidence of mild traumatic brain injury (mTBI) has remained high and has become a worldwide public health problem. There is a pressing need for better practice guidelines in clinic to standardize mTBI clinics, thereby providing mTBI patients with a timetable to return to work safe. Imaging studies are not yet sensitive to the diagnosis of mTBI, and the development of a set of biomarkers that can reflect the underlying pathophysiology of mTBI, that will contribute to clinical diagnosis and prognostic of mTBI. Based on the pathological anatomy and pathophysiology of mTBI, this article systematically elaborates the advancements of blood-brain barrier disruption, axonal injury, neuroinflammation, and gene biomarkers, in order to discover some promising biomarkers that are availible in clinic and promote mTBI objective diagnosis and personalized treatment.

Research progress of inflammation related signal transduction pathway in atherosclerosis

Atherosclerosis (AS) is the pathological basis of cardiovascular disease, which is a serious threat to human health. In recent years, more and more scholars pay attention to the theory of inflammation. It is a new idea to treat AS from the point of view with antiinflammation and immunoregulation. There are multiple signal transduction pathways involved in the course of inflammation progress. The signaling pathway is studied deeply that is expected to become a target for intervention of atherosclerosis drugs. This article summerizes the prospective basic experimental from cellular and molecular level, to bring about inspiration in the treatment of AS and AS related diseases.

Diagnosis criteria and screening strategy of metabolic syndrome in children and adolescents

The incidence of metabolic syndrome and obesity in children and adolescents has increased gradually. As the definition of metabolic syndrome has not been unified, the diagnosis of children and adolescents metabolic syndrome is still controversial. Although the pathogenesis is still unclear, insulin resistance and subsequent inflammation play an important role in the pathogenesis. The main clinical manifestations of metabolic syndrome are overweight and obesity, abnormal glucose metabolism, dyslipidemia, and hypertension. Fatty liver, polycystic ovary syndrome and pro-inflammatory state have close correlation with metabolic syndrome. Regular screening of metabolic syndrome should be carried out for children and adolescents with high risk factors.

Research progress of drug therapy in the treatment of knee osteoarthritis

Knee osteoarthritis is a common chronic degenerative disease of knee joint. In the advanced stage, the joint may appear to bend and stretch limitation, crouch difficult, and some people even can′t walk when it′s serious. Drug treatment plays an important role in the clinical treatment of knee osteoarthritis, but rational drug use has not received much attention. Drug treatment include oral and topical drugs. Most patients take anti-inflammatory and analgesic drugs for a long time, and they are not aware of the indications and side effects of their treatment; some patients do not know enough about the application of external drugs. Oral drugs include analgesics, cartilage protection drugs; intra-articular injection of drugs include hyaluronic acid, sodium vitreate, medical chitin, hormone drugs, autorich platelet plasma, ozone, etc. In addition, there are also topical drugs. This article mainly summarizes the drugs for the treatment of knee osteoarthritis, in order to guide the clinical medication better and improve the therapeutic effect of knee osteoarthritis.

Study on the status of cardiopulmonary resuscitation of residents of Mingyuan Community in Wenzhou City

Objective To understand the cognitive status of cardiopulmonary resuscitation (CPR) among community residents in Wenzhou City, and to provide reference for the promotion and popularization of national CPR. Methods Using a random cluster sampling method and using a self-designed CPR questionnaire, an on-site survey was conducted on 330 resident populations of the Mingyuan Community over the age of 18 who met the inclusion criteria from January to March 2017, and the status quo of knowledge, attitude, practice (KAP) was analyzed by four-quadrant analysis. Results A total of 330 people were investigated, 300 valid questionnaires were collected, and the effective rate was 90.90%. The awareness rate of first aid knowledge of CPR was 40.00% - 97.50%, and the rate of correct attitude was 42.10% - 89.29%, correct behavior holding rate was 12.50% - 50.00%. Four quadrant analysis showed that the proportion of population falling into the first quadrant (knowledge behavior are correct), the second quadrant (incorrect knowledge, and correct behavior), the third quadrant (knowledge behavior are not correct), the fourth quadrant (incorrect behavior, and correct knowledge) was 46.33%, 26.33%, 19.33%, 13.33% respectively. Conclusion The community has a low awareness of CPR and a strong willingness to learn. Therefore, it is necessary to effectively strengthen the CPR knowledge and skills training of the community, so as to improve the rescue success rate of patients with cardiac arrest.

Investigation on lifestyle and awareness status of diabetes among primary and secondary school students in Shanghai suburb

Objective To investigate the diabetic cognitive situation, lifestyle in Shanghai suburb elementary and middle school students and analysis the related influence factors. Methods From October to November 2015, 174 suburban preparatory classes students and 88 fifth grade students in the same area middle school were selected to carry out the questionnaire survey, the cognition of diabetes, bad lifestyle and its influence factors were investigated and statistically analyzed. Results The overall awareness rate of diabetes and diabetes prevention in primary and secondary school students were 84.4% and 50.4%. The percent of students who had poor lifestyle was 88.2%, the percent of overweight and obesity was 25.6%. Among them 77.27% pupils like drinking beverage and 73.61% secondary school students like eating fired food. Single factor Logistic regression analysis showed that the family history of diabetes was the influencing factor of awareness rate of diabetes and diabetes prevention and participation in preventive action among primary and secondary school students in Shanghai suburb, and age was the influencing factor of awareness of diabetes, and the awareness rate of diabetes prevention was the influential factor in the prevention of diabetes action (P < 0.05). Conclusion The proportion of primary and secondary school students poor lifestyle with in Shanghai suburb is much higher and most of them lack of the awareness of obesity and diabetes. It is necessary to carry out diabetes health education among them and develop the ability of diabetes intervention.

Relationship between serum creatinine level and clinical indicators in middle-aged patients with type 2 diabetes mellitus

Objective To explore the relationship between serum creatinine level and clinical indicators in middle-aged patients with type 2 diabetes mellitus, and provide evidence for better prevention of such complications in clinic. Methods Clinical data of 183 type 2 diabetes mellitus (T2DM) patients with complete data records and ≥45 years old, admitted to the Department of Endocrinology, Beijing Aerospace General Hospital from January to December 2017. All the cases were divided into two groups according to the level of serum creatinine. Group A was normal serum creatinine group (n = 126), the serum creatinine was ≤104 μmol/L, and group B was serum creatinine elevated group (n = 57), serum creatinine was >104 μmol/L. Age, body mass index (BMI), diabetes course, fasting blood glucose (FPG), glycosylated hemoglobin (HbA1c), triglyceride (TG), total cholesterol (TC), low density lipoprotein cholesterol (LDL-C) and high density lipoprotein cholesterol (HDL-C), serum uric acid (UA), urine microalbumin (UMA) of the two groups were collected and analyzed. Results Single factor analysis showed that, age, course of diabetes, FPG, HbA1c, TG, TC, UA, UMA had statistically significant difference between the two groups (P < 0.05). Multiple Logistic regression analysis showed that, the course of diabetes mellitus, HbA1c, TC, UA, UMA were independent risk factors for the increase of serum creatinine (P < 0.05). Conclusion The patient′s diabetes course, HbA1c, TC, UA and UMA are the risk factors for increase of serum creatinine among the middle-aged patients with type 2 diabetes mellitus.

Efficacy of Finasteride in the treatment of castration resistant prostate cancer

Objective To study and discuss the efficacy of Finasteride in the treatment of castration resistant prostate cancer. Methods Clinical data of 72 patients with castrated resistant prostate cancer admitted to Liuzhou Worker's Hospital from January 2010 to January 2016 were collected to study retrospectively. They were divided into control group (Flutamide treatment group) and observation group (Flutamide and Finasteride treatment) according to different treatment therapy, with 36 cases in each group. Clinical efficacy, changes of urodynamics (MFR, UFR and PVR), serum prostate specific antigen (PSA), volume of prostate and tumor were compared between the two groups. And the PSA progression-free survival time, total survival time, quality of life score and adverse reaction were also compared between the two groups. Results Six months after treatment, the effective rate of the observation group was higher than that of the control group (72.2%) (P < 0.05). The reductions of MFR, UFR and PVR of the observation group were higher than those of the control group (P < 0.05). The PSA level of the two groups significantly declined (P < 0.05), and the lowest PSA level in the observation group was lower than that in the control group (P < 0.05). The proportion of patients with normal PSA level in the observation group was significantly higher than that in the control group (P < 0.01). The prostate and tumor volume in the observation group were smaller than those in the control group (P < 0.01). Followed up for 6-24 months, survival analysis showed that the median PSA progression-free survival and total survival time of the observation group were longer than those in the control group (P < 0.05). The scores of quality of life in the observation group were higher than those of the control group (P < 0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion The effect of Finasteride is significant in the patients with castration resistant prostate cancer. It can obviously improve the urodynamics and PSA level, inhibit the development of tumor, and prolong the survival time and improve quality of life.

The application value of ultrasound combined with multi-slice spiral CT angiography in nephron sparing surgury for small renal cancer

Objective To investigate clinical value of ultrasound combined with multislice spiral CT angiography (MSCTA) in nephron sparing surgery (NSS) for small renal cancer. Methods Based on the usage of ultrasound combined with MSCTA, 65 cases diagnosed with small renal cell carcinoma from May 2014 to May 2016 in the Affiliated Hospital of Chengde Medical College were divided into two groups, experimental group and control group respectively. The experimental group was consisted of 33 cases and the control group included 32 cases. The following indicators were compared between the two groups: operative time, intraoperative blood loss, intraoperative thermal ischemia time, the small lesion detection rate during operation, postoperative time to remove the peritoneal drainage tube, average hospital stay after surgery, postoperative complication rate, positive rate of postoperative pathological margin, postoperative GFR of surgical kidney. Results Intraoperative findings of renal vascular variation rate was consistent with that of preoperative MSCTA. In the experimental group, one case of small lesions was revealed by intraoperative ultrasonography. One case of postoperative urinary leakage occurred in the control group. The operative time, intraoperative blood loss, intraoperative thermal ischemia time, postoperative time to remove the peritoneal drainage tube, positive rate of postoperative pathological margin, average hospital stay after surgery were less in the experimental group than the control group (P < 0.05). GFR before and after surgery in the experimental group had no statistically significant difference (P > 0.05). The postoperative GFR of the control group was significantly lower than that before surgery (P < 0.05). Conclusion MSCTA combined with intraoperative ultrasonography control renal vessels more rapidly and reduce the risk of surgery and postoperative complications. The kidney units can also be retained to the maximum extent. It has good prognosis and great value in clinical application.

Analysis of the correlation between bone cement dispersion type and recurrent vertebral collapse after vertebroplasty

Objective To investigate the correlation between bone cement dispersion type and recurrent vertebral collapse after vertebroplasty (PVP/PKP) recurrent vertebral collapse after vertebroplasty. Methods Clinical data of 411 patients admitted to Guangdong Province Traditional Chinese Medical Hospital treated with vertebroplasty from April 2009 to May 2016 was analyze retrospectively. Postoperative patients all underwent X-ray examination and were divided into collapse group and non-collapse group according to the follow-up results. Univariate analysis was performed to compare the two groups in terms of gender, age, vertebral compression rate, vertebral height recovery rate, postoperative cobb angle correction value, bone cement injection volume, bone cement dispersion type, bone cement distribution area, and cement leakage. For the differences, Logistic regression analysis was performed. Results All the patients were followed up. The followed up time was from 1 to 37 months with a mean of (17.60±7.28) months. There were 59 cases occured vertebral collapse with the incidence of 14.36%. Vertebral compression rate, the vertebral height recovery rate, bone cement diffusion type, and bone cement leakage were significantly different between the two groups (P < 0.05). The multiple Logistic regression analysis showed that, bone cement with poor dispersion in the fracture line, bone cement with a dense distribution, the vertebral anterior height recovering excessively, PKP were the risk factors for vertebral re-collapse after vertebroplasty (P < 0.01). Conclusion The excessive recovery of the anterior vertebral height, the poor dispersion of bone cement within the fracture line, and the dense distribution of bone cement, PKP are independent risk factors for the recurrence of the vertebral surgery after vertebroplasty.

Effect comparison of radial divergent extracorporeal shock wave and traditional method in the treatment of plantar fasciitis

Objective To observe whether the clinical effect of radial extracorporeal shock wave therapy for plantar fasciitis is better than conventional treatment therapy. Methods A total of 60 patients underwent treatment continuously from July 2016 to March 2017 were included in this study. All patients were randomly divided into radial extracorporeal shock wave group and traditional treatment group (control group), with 30 cases in each group. Patients in radial extracorporeal shock wave group underwent radial extracorporeal shock wave, with pressure of 2.5-4.0 bar, 2000 impacts each time, one time per week and 3 times totally. The control group was treated with topical solution for Flurbiprofen Cataplasms combined with plantar fascia stretching exercise. The visual analogue scale (VAS) of the two groups was compared before treatment, and 1, 4, 12 and 24 weeks after treatment. The AOFAS scores before treatment and at the end of follow-up were compared between the two groups. The overall superior rate were compared between the two groups after treatment for 24 weeks. Results The VAS scores in the shock wave group were all lower than those in the control group after treatment for 1, 4, 12, and 24 weeks, and the differences were statistically significant (P < 0.05). The function score of shock wave group was higher than that of the control group at 24 weeks′ follow-up, and the difference was statistically significant (P < 0.05). After treatment for 24 weeks, the overall superior rate of the shock wave group was also higher than that of the control group, and the difference was statistically significant (P < 0.05). Conclusion Both therapies can reduce pain and improve the ankle functions for plantar fasciitis patients, but the effect of radial extracorporeal shock wave is significantly better than the traditional treatment of non steroidal anti-inflammatory drugs combined with plantar fascia stretching training, is a new choice for the treatment of plantar fasciitis.

Objective To evaluate the clinical value of ultrasonography in the diagnosis of axillary lymph node metastasis in breast cancer. Methods Clinical data of 682 primary invasive breast cancer patients who underwent treatment in Breast Center, Shunyi Women′s and Children′s Hospital of Beijing Children′s Hospital from January 2008 to July 2017 were studied retrospectively. Patients were divided into normal group (n = 419) and abnormal group (n = 263) depending on the morphology and cortical thickness of axillary lymph nodes through ultrasonography. The patients in the abnormal group underwent ultrasound guided fine needle aspiration (FNA). Except for patients who were diagnosed with positive axillary lymph nodes, the others underwent sentinel lymph node biopsy (SLNB). Two hundred and fifteen of the 263 cases which had abnormal lymph node diagnosis were confirmed positive by histopathology, and 99 of 419 cases which had normal lymph node diagnosis were confirmed positive by histopathology. Histopathological diagnosis was set as gold standard, the value of ultrasound classification for axillary lymph nodes was evaluated. Kappa analysis was used to evaluate the consistency of ultrasonography and histopathology. Pearson χ2 test was used to analyze the related factors of axillary lymph node metastasis. Results The sensitivity, specificity, positive and negative predictive value of ultrasonography in the diagnosis of axillary lymph node metastasis in breast cancer was 68.5%, 87.0%, 81.7%, 76.4% respectively. The ultrasonography and histopathology was in good coincidence, and the value of Kappa was 0.561. The axillary lymph node metastasis in breast cancer was related to family history of the patients (P < 0.05), but not significantly related to age, menopausal status or subtype classification (P > 0.05). Conclusion Ultrasonography has a higher clinical practical diagnostic value for axillary lymph node metastasis in breast cancer. The axillary lymph node metastasis is related to family history, but not significantly related to age, menopausal status or subtype classification.

Study of combined multiple methods to prevent ovarian hyperstimulation syndrome

Objective To explore the efficacy of combined multiple methods in preventing ovarian hyperstimulation syndrome during the in vitro fertilization-embryo transfer (IVF-ET) cycles. Methods From January 1st, 2016 to December 31st, 2017, a total of 120 OHSS high-risk patients with IVF-ET in Zhongnan Hospital of Wuhan University were included in the multi-method prevention group. In addition to the selective cryopreservation of embryos, calcium gluconate, cabergoline, infusion of albumin, GnRH antagonists and other methods were added in luteal phase. In addition, 95 high-risk OHSS patients who underwent IVF-ET from January 1st, 2014 to December 31st, 2015 were included in the control group. The primary outcome measure was the incidence of OHSS, and the secondary outcome measure was the days needed to relieve symptoms. Results The incidence of severe OHSS in control group and experimental group was 6.3%(6/95) and 0.8% (1/120), respectively. There was a significant difference between the two groups (P < 0.05). The days required for remission of signs in severe OHSS patients in the control group and experimental group were (6.8±2.1) and (5.4±1.8) d, respectively, and there was a statistically significant difference between the two groups (P < 0.05). However, there was no statistically significant difference in days needed to relieve symptoms of mild OHSS patients (P > 0.05). Conclusion The combination of multiple methods to prevent ovarian hyperstimulation syndrome is scientific and effective, which provides a powerful guide for the prevention of OHSS in IVF-ET.

The level changes of tau protein, activin A, neuro-specific enolase and its clinical significance in premature infants with hypoxic ischemic encephalopathy

Objective To study the level changes of tau protein, activin A, neuro-specific enolase (NSE) in premature infants with hypoxic ischemic encephalopathy, and to analyze its clinical significance. Methods The total of 131 cases of hypoxic ischemic encephalopathy treated in Xuzhou Children′s Hospital from February 2016 to December 2017 were selected as the study group, 100 cases of premature infants without hypoxic ischemic encephalopathy at the same period were selected as the control group. The serums tau protein, activin A and NSE levels of postnatal 7 d infants in two groups were compared. The preterm infants in the study group were divided into mild group (32 cases), moderate group (61 cases) and severe group (38 cases) according to the severity of the disease. The serum levels of tau protein, activin A, NSE and neonatal neurobehavioral (NBNA) score in postnatal 7 d infants with different severity was compared. The correlation between NBNA score, serum tau protein, activin A and NSE level was also analyzed. Results The levels of serum tau protein, activin A and NSE in preterm infants of the study group were significantly higher than those of the control group, the differences were statistically significant (all P < 0.01). Serum tau protein and activin A and NSE levels of mild group, moderate group and severe group showed a rising trend, the differences between groups were statistically significant (all P < 0.01). The NBNA score in the preterm 7 d infants of severe group was significantly lower than that of the moderate group and mild group, while the moderate group was significantly lower than that of the mild group, the differences between the groups were statistically significant (P < 0.05). Pearson correlation analysis showed that serum tau protein, activin A and NSE levels in premature infants with hypoxic ischemic encephalopathy were negatively correlated with NBNA scores (r = -0.612, -0.583, -0.648, all P < 0.05); the level of serum tau protein in premature infants with hypoxic ischemic encephalopathy was positively correlated with the levels of activin A and NSE (r = 0.531, 0.487, all P < 0.05); serum activin A was positively correlated with NSE level (r = 0.443, P < 0.05). Conclusion The serum tau protein, activin A and NSE of hypoxic-ischemic encephalopathy are highly expressed, and are closely related to the neural development of premature infants.

Effect of Yangxin Dingji Capsules on related hormone and neurotransmitter in patients with perimenopausal syndrome

Objective To discuss the effect of Yangxin Dingji Capsules on clinical symptoms, related hormone and neurotransmitter in patients with perimenopausal syndrome. Methods From November 2014 to July 2016, 104 patients with perimenopausal syndrome treated in the Third Hospital of Shijiazhuang City were divided into control group and study group by random number table, with 52 cases in each group. Patients in control group were treated with Nilestriol, patients in the study group were treated with Yangxin DingJi Capsules and Nilestriol. Treatment lasts for 4 weeks. Before and after treatment, clinical effects were compared between two groups. Kupper-man score, menopause rating scale (MRS), self-rating depression scale (SDS), and serum estradiol (E2), follicle-stimulating hormone (FSH), luteinizing hormone (LH), 5-hydroxytryptamine (5-HT), norepinephrine (NE) and dopamine (DA) levels of the patients in two groups were measured. Adverse reactions were recorded in two groups. Results The total effective rate in study group was 90.38%, which was significantly higher than those in control group (P < 0.05). Compared with before treatment, the Kupper-man, MRS, SDS scores of the two groups were significantly lower after treatment, and the study group was significantly lower than that in control group (P < 0.01); the level of E2 was significantly higher, levels of FSH and LH were significantly decreased, and the study group improved more significantly (P < 0.01); the levels of 5-HT, NE and DA increased significantly, and that in study group was significantly higher than in control group (P < 0.01). Conclusion Yangxin Dingji Capsules can significantly improve the clinical symptoms, hormone and neurotransmitter levels of patients with perimenopausal syndrome and high security.

The influence of glucocorticoid combined with immunoglobulin on infantile pneumonia

Objective To approach influence of glucocorticoid combined with immunoglobulin on infantile pneumonia. Methods The clinical data of 100 cases of infantile pneumonia in Department of Pediatrics of Yan'an People′s Hospital from January 2015 to January 2018 were retrospectively analyzed, and they were divided into control group and observation group by different treatment methods, with 50 cases in each group. The children with pneumonia of two groups were treated with anti-inflammatory, atomized and antitussive therapy, the control group was given intravenous injection of gamma globulin in addition, and the observation group was given intravenous injection of methylprednisolone on the basis of the control group. The changes of WBC, C-reactive protein, procalcitonin of two groups before and after treatment were observed. The antifebrile time, cough time, hello time, shadow absorption time, hospital stays, and clinical treatment total effective rate of two groups were observed. Results The WBC, C-reactive protein and procalcitonin of two groups before treatment were compared, the differences were no statistical significance (P > 0.05). After treatment, the WBC, C-reactive protein, procalcitonin of two groups were lower than the those before treatment, and those of the observation group were lower than the control group, the differences were statistical significance (P < 0.05). The antifebrile time, cough suppressant time, rale disappear time, chest shadow absorption time and hospital stays of observation group were shorter than those of the control group, the differences were statistical significance (P < 0.05). The treatment total effective rate of the observation group was higher than that of the control group, the difference was statistical significance (P < 0.05). Conclusion The glucocorticoid combined with immunoglobulin in the treatment of infantile pneumonia, can decrease serum inflammatory markers, shorten clinical symptom improvement time, improve clinical treatment result.

Clinical significance of VEGF, Eotaxin and Mucin MUC5AC expression in patients with chronic rhinosinusitis and nasal polyps before and after endoscopic surgery

Objective To investigate the level change and clinical significance of the expressions of eosinophils chemokines (Eotaxin), mucin MUC5AC and vascular endothelial growth factor (VEGF) in patients with chronic rhinosinusitis and nasal polyps before and after endoscopic surgery. Methods From February 2013 to December 2016 in Shanghai Fengxian District Central Hospital, the 62 cases with chronic sinusitis and nasal polyps treated with endoscopic surgery were selected as the observation group, and 62 cases of hospitalized patients with deviation of nasal septum treated with septum rectifying operntion were selected as the control group. The HE staining result, the infiltration of eosinophils, the expressions of Eotaxin, mucin MUC5AC and VEGF were observed and compared between the two groups before and after operation. Results The eosinophil count in the observation group was significantly less than that before operation, and the difference was statistically significant (P < 0.01). Compared with the control group, the eosinophil count was higher in the observation group before and after operation, and the difference was statistically significant (P < 0.01). The expression percentage of Eotaxin, mucin MUC5AC and VEGF was significantly lower in the observation group than those before operation, and the differences were statistically significant (P < 0.01). Compared with the control group, the percentages of Eotaxin, mucin, MUC5AC and VEGF expression were higher in the observation group before and after the operation, and the differences were statistically significant (P < 0.01). Conclusion The Eotaxin, mucin, MUC5AC and VEGF in the chronic rhinosinusitis and nasal polyps patient are over expressed, and they are closely related to the changes of disease condition.

Comparison of clinical effects of three subgingival irrigation as adjunctive therapies in the treatment of chronic periodontitis

Objective To investigate and analyze the clinical efficacy of three types of subgingival irrigation as adjunctive therapies in the treatment of chronic periodontitis. Methods A total of 400 maxillary first molar of patients with chronic periodontitis in the Aviation General Hospital and Beijing Chuiyangliu Hospital from June 2016 to June 2017 were selected and divided into hydrogen peroxide group, Iodophor group, hydrogen peroxide + Iodophor group and NS group, with 100 patients in each group. After subgingival scaliing and or root planning, teeth to all the subjects, patients in hydrogen peroxide group, Iodophor group, and hydrogen peroxide + Iodophor group were irrigated by hydrogen peroxide, Iodophor, hydrogen peroxide + Iodophor, respectively, and the NS group was irrigated with 0.9% NS as the control. Once a week, for 4 weeks. Parameters including gingival index (GI), bleeding index (BI), attachment level (AL) and probing depth (PD) were compared before and after treatment. Results After treatment, GI, BI, AL and PD of patients in the four groups were significantly reduced, and the PD and AL of the hydrogen peroxide + Iodophor group were significantly lower than those of the other three groups (P < 0.05). After treatment, there was no significant difference in GI or BI among the four groups (P > 0.05). Conclusion All the three types of subgingingival irrigation can improve the curative effect to different degrees, and compared with other simple subgingival irrigating solutions, hydrogen peroxide and Iodophor achieves better clinical effectiveness.

The study of microbial limit test method for different Chinese medicine preparations

Objective To establish the microbial limit test for 9 kinds of traditional Chinese medicine preparations. Methods According to the Chinese Pharmacopeia 2015 edition 4th part general rule 1100 biological examination method, the microbial limit test method was established and verified. Results By using normal method, medium dilution method and membrane filtration method respectively, the recovery rate of each test bacteria sample was both up to 0.5-2.0, which met the Chinese Pharmacopeia requirements. Conclusion The established method is accurate and reliable, which can be used for the microbial limit test of 9 kinds of traditional Chinese medicine preparations.

Content determination of sialic acid in recombinant human erythropoietin by high-performance anion-exchange chromatography-pulsed amperometric detection

Objective To develop a high-performance anion-exchange chromatography-pulsed amperometric detection (HPAEC-PAD) method to determine the content of sialic acid in recombinant human erythropoietin (rhEPO). Methods The rhEPO samples were prepared by changing rhEPO bulks to 5 mmol/L phosphate buffer (pH 7.0), and adjusted to 0.33 mg/mL. Taking 20 μL sample solution, and then 20 μL of 10 mU/μL neuraminidase was added, followed by incubation at 37℃ for 1 h. Finally, 50 μL 0.04 mmol/L ketodeoxynonulosonic acid (KDN) and 70 μL milli-Q water were added and mixed thoroughly followed by detection. CarboPac PA1 separation column and CarboPac PA100 guard column were used, with mobile phase being mixture of 150 mmol/L sodium hydroxide solution and 125 mmol/L sodium acetate solution, column temperature being 30℃, flow rate being 1.0 mL/min, and the data acquisition time being 15 min with isocratic elution. The sialic acid content was calculated based on the dose-effect relation of the ratio of peak area of sialic acid to KDN and the content of sialic acid. Results The concentrations of sialic acid standard solutions and the ratios of peak area of sialic acid to KDN showed a good linearity relationship between 0.0015 and 0.0075 μg/mL with R2 being higher than 0.99, the repeatability RSD was below 4%. The recovery rates of samples at low, medium and high concentration were 94.9%, 94.3% and 96.7%, respectively. Six batches of rhEPO were tested by the developed HPAEC-PAD method and the results had no significant differences compared with the results of resorcinol method (P = 0.7488). Conclusion This research has successfully established the HPAEC-PAD method to determine the content of sialic acid in rhEPO, with good linearity, repeatability and robustness, which can be used in routine determination of sialic acid in rhEPO.

Effect of Tigecycline in the treatment of severe pneumonia caused by multidrug resistant bacteria and its influence on oxygen metabolism state

Objective To investigate the effect of Tigecycline in the treatment of severe pneumonia caused by multidrug resistant bacteria and its influence on oxygen metabolism state. Methods From October 2016 to November 2017, 92 patients diagnosed as severe pneumonia caused by multidrug resistant bacteria in Dalian Municipal Central Hospital were selected and divided into control group and study group according to the different treatment methods, with 46 cases in each group. The control group was treated with Cefoperazone-Sulbactam by intravenous drip, the study group was treated with Tigecycline by intravenous drip on the basis of treatment of control group. The treatment of two groups lasted for 3 d. The curative effects of the two groups were observed and compared after treatment, the oxygen metabolism state (oxygen supply and oxygen consumption state) of two groups were detected 1 d before treatment and 3 d after treatment, and the adverse reactions rates of the two groups during the treatment were compared. Results After treatment, the curative effect of study group was significantly better than that of control group, the difference was statistically significant (P < 0.05). The oxygen supply (DO2) and oxygen supply index (DO2I), oxygen consumption (VO2) and oxygen consumption index (VO2I), arterial oxygen pressure (PaO2) and arterial oxygen saturation (SaO2) of the two groups 3 d after treatment were higher than those before treatment, the differences were statistically significant (P < 0.05), and the detection values of DO2, DO2I, VO2, VO2I, PaO2 and SaO2 of study group 3 d after treatment were significantly higher than those of control group, the differences were statistically significant (P < 0.05). There were no significant differences in the rates of adverse reactions including diarrhea, nausea, vomiting and rash during treatment between the two groups (P > 0.05). Conclusion Tigecycline is effective in the treatment of patients with severe pneumonia caused by multidrug resistant bacteria, which is helpful to improve the oxygen metabolism, and there is no obvious increase in adverse reactions, so it is worthy of promotion and application.

Clinical efficacy of meridian guide flat combined with Mecobalamin in the treatment of diabetic peripheral neuropathy

Objective To investigate the clinical effect of meridian guide flat combined with Mecobalamin in the treatment of diabetic peripheral neuropathy. Methods Eighty patients with diabetic peripheral neuropathy admitted in Wangjing Hospital of Chinese Academy of Traditional Chinese Medicine and Yongledian Community Health Service Center of Tongzhou District in Beijing City from May 2016 to February 2017 were selected and they were divided into control group and treatment group according to random number table method, with 40 cases in each group. The control group was treated with Mecobalamin simply, while the treatment group was treated with Mecobalamin combined with meridian guide flat. The two groups were treated for 8 weeks. Clinical symptoms, conduction velocity of sensory and motor nerves, blood rheology were compared between the two groups before and after treatment. Results The total effective rate of treatment group was higher than that of control group, the difference was statistically significant (P < 0.05). Motor and sensory conduction velocity of common peroneal nerve and median nerve were higher after treatment in both groups, and treatment group was significantly higher than control group (P < 0.05). Whole blood high, medium, low shear viscosity and plasma viscosity declined after treatment in both groups(P < 0.05), and those in the treatment group were significantly lower than control group (P < 0.05). Conclusion Meridian guide flat combined with Mecobalamin can relieve symptoms of patients with diabetes peripheral neuropathy and improve conduction velocity of sensory and motor nerves, it also can improve blood rheology and have satisfied effect.

Effect of Huoxue Ointment combined with herbal fumigation in the treatment of knee osteoarthritis and its influence on IL-1β and MMP-9

Objective To analyze the effect of Huoxue Ointment combined with herbal fumigation in the treatment of knee osteoarthritis (KOA) and its influence on interleukin-1 beta (IL-1β) and matrix metalloproteinase (MMP-9). Methods Ninety-two patients with KOA admitted to the People′s Hospital of Liuzhou City of Guangxi Zhuang Autonomous Region from September 2015 to August 2017 were selected and randomly divided into the control group and the observation group by envelope method, with 46 cases in each group. The control group was treated with self-made Huoxue Ointment applicator therapy, and the observation group was treated with Huoxue Ointment and Chinese herbal fumigation. After continuous treatment for 2 weeks, the differences of curative effect, levels of IL-1β and MMP-9, visual analogue scale (VAS) score, hospital specialsurgery (HSS) knee score, short form 36 questionnaire (SF-36) score and adverse reactions between the two groups were compared. Results After treatment, the total effective rate of observation group (97.83%) was higher than that of the control group (84.78%), there was a statistically significant difference between the two groups (P < 0.05). After treatment, the levels of serum IL-1 and MMP-9 of the two groups were significantly lower than those before treatment, and the levels of IL-1 and MMP-9 in the observation group were lower than those in the control group, the differences were statistically significant (P < 0.05). After treatment, the VAS scores of the two groups were all lower than those before treatment, while the HSS scores and SF-36 scores of the two groups were all higher than those before treatment, the differences were statistically significant (P < 0.05), and the VAS score of observation group was lower than that of control group, the HSS score and SF-36 score were higher than those of control group, the differences were statistically significant (P < 0.05). There was no significant difference in the incidence of adverse reactions between the two groups during the treatment period (P > 0.05). Conclusion Combined application of Huoxue Ointment and herbal fumigation in the treatment of knee osteoarthritis has satisfactory curative effect, which can effectively decrease the levels of serum inflammatory factors, reduce the pain, improve the knee joint activities and quality of life.

Analysis of professor Zheng Meisheng′s experience in treating viral myocarditis from spleen

Viral myocarditis refers to localized or diffusive myocardial inflammation inducd by virus infection. Traditional Chinese medicine has achieved certain curative effect in the diagnosis and treatment of this disease. Spleen and stomach are acquired foundation. The dysfunction of spleen and stomach leads to deficiency of vital qi, invasion of pathogenic factors, accumulation of sputum, turbid water and other pathological products, which affects the body′s qi, blood, yin and yang, thus the root is the disorder of spleen and stomach. On the basis of holistic treatment, professor Zheng Meisheng always pays attention to regulating spleen and stomach, invigorating the spleen, and benefiting primordial qi during the process of treatment based on syndrome differentiation. An analysis of the prospect for traditional Chinese medicine development in Russia under the Belt and Road Initiative

Russia is an important country along the Belt and Road whose history of disseminating traditional Chinese medicine (TCM) dates back to the 18th century. After three centuries of development, TCM has been widely recognized in Russia, with a large number of experts and scholars continuously studying it and many works published. Acupuncture has even developed into acupuncture and reflexology with Russian characteristics. In order to further promote TCM development in Russia, it is necessary to study the problems facing TCM at the moment in an objective manner, such as academic misconduct, difficulties in drug registration and illegal medical practices. The support of local laws and policies is a key guarantee for the healthy and sustainable development of TCM in Russia. Therefore, it is necessary to closely cooperate with local authorities to promote TCM legislation. New paths and models should be explored on that basis by participating in multilateral cooperation of international organizations and joining hands with local enterprises for common development and shared benefits. This is also in line with the principles of policy coordination, unimpeded trade, and closer people-to-people ties, as proposed in the Belt and Road Initiative.

Application of modern 3S technique to investigate the stock of wild genuine regional drugs in Handan City and comprehensive evaluation on its current situation

Objective Modern 3S technology was applied to investigate the stock of 24 kinds of wild genuine regional drugs in Handan City, such as Radix Bupleuri, Rhizoma Anemarrhenae, etc, to evaluate its current situation and provide a basis for the protection of wild genuine regional drugs. Methods 3S technology was used to investigate the stock of wild genuine regional drugs in Handan City and to calculate its stock, the status of wild genuine regional drugs was evaluated by analytic hierarchy process. Results Among the 24 species of wild genuine regional drugs, there were 3 species of stock more than 1000 tons, 12 species between 100 tons and 1000 tons, and 9 species less than 100 tons. Through comprehensive evaluation, it was found that 6 kinds of traditional Chinese medicine, such as Semen Ziziphi Spinosae and Herba Leonuri, had a good comprehensive status. 10 kinds of traditional Chinese medicine, such as Spina Gleditsiae, Radix Sophorae Flavescentis, had a general comprehensive status. The comprehensive present situation of 8 kinds of traditional Chinese medicine, such as Rhizoma Polygonati and Rhizoma Anemarrhenae, was not optimistic, and should be paid more attention. Conclusion The stock of Handan genuine regional drugs is large, but the difference among different varieties is very large, so it is necessary to carry out reasonable development and utilization and protection under the full understanding of the background.

The treatment of Parkinson′s disease from phlegm and blood stasis

The pathogenesis of Parkinson′s disease is deficiency, and viscera weak can produce the phlegm and blood stasis, so phlegm and blood stasis is the important pathologic factor. The phlegm and blood stasis is the pathological production of the viscera, and the phlegm and blood stasis are blocked in the meridians, the brain and muscles can't be nourished, then lead to movement disorder, body tremor and so on. Because of the deficiency, blood stasis appears, because of the blood stasis, the deficiency is more serious, if the blood stasis can't be moved, the deficiency is difficult to repair. Therefore, removing blood stasis and eliminating phlegm is very important in the treatment of Parkinson′s disease, and the application of removing blood stasis and eliminating phlegm medicine in treatment of Parkinson′s disease should be taken seriously in clinic.

Effect of acupotomy lysis with tiaoji tongdu on regulating lumbar flexion activity in patients with lumbar disc herniation

Objective To investigate the effect of acupotomy lysis with tiaoji tongdu on regulating lumbar flexion activity in patients with lumbar disc herniation (LDH). Methods Ninety-six patients with LDH admitted to the Department of Acupuncture of Guanggu Branch of Hubei Provincial Hospital of Traditional Chinese Medicine from November 2016 to December 2017 and met the inclusion criteria were divided into the needle-knife group and the electroacupuncture group according to the random number table method, with 48 cases in each group. Finally, 46 cases of needle-knife group and 45 cases of electroacupuncture group finished the study. The needle-knife group was treated by the acupotomy lysis with tiaoji tongdu method, once every 5 days, once for 1 course of treatment. Electroacupuncture group was treated by simple electroacupuncture, once a day, continuous treatment for 4 days for a course of treatment, both groups were treated for 3 courses, and followed up for 2 weeks. The clinical efficacy of the two groups and changes of lumbar flexion activity before treatment, after 3 courses of treatment and follow-up visit, and the rate of recurrence of two groups were observed and compared. Results After 3 courses of treatment, the total effective rate between the two groups had no statistically significant difference (P > 0.05). There was no significant difference in the lumbar flexion activity between the two groups before treatment (P > 0.05), the lumbar flexion activity scores of the two groups after treatment for 1, 2, 3 courses were higher than those before treatment (P < 0.05), and the needle-knife group was significantly higher than that of the electroacupuncture group, the difference was statistically significant (P < 0.05). After 2 weeks of follow-up, the recurrence rate of the needle-knife group was significantly lower than that of the electroacupuncture group, the difference was statistically significant (P < 0.05). Conclusion The needle-knife with tiaoji tongdu therapy and electroacupuncture therapy have certain curative effect on improving lumbar flexion activity in LDH patients, but needle-knife therapy has better improvement, which can significantly improve waist flexion activity and achieve faster effect. It is more effective and easier to operate and saves time, which is worthy of clinical promotion.

Clinical efficacy of different needle-knife target selection in the treatment of cervicogenic headache

Objective To observe the clinical effect of different needle-knife target selection in the treatment of cervicogenic headache. Methods Sixty patients with cervicogenic headache from April 2015 to December 2016 in Oupatient Department of Acupuncture and Moxibustion of the Second People′s Hospital of Taixing City, Nanjing Hospital of Chinese Medicine and Daishan Clinical Center of Nanjing Hospital of Chinese Medicine were selected as research objects. The patients were divided into needle-knife group 1 (taking the starting point and dead point of the cervical muscle as the entry points) and needle-knife group 2 (taking the points 2 cm away from 2, 3, 4 cervical spinous as the entry points) according to random number table method, with 30 cases in each group. The treatment effect, visual analogue scale (VAS), neck pain score of two groups were compared. Results After treatment for 2 weeks and 3 months, the VAS scores and neck pain scores of two groups decreased significantly compared with those before treatment (P < 0.05), and the VAS score and neck pain score of needle-knife group 1 after treatment for 3 months decreased significantly compared with those after treatment for 2 weeks (P < 0.05). And the score of neck pain of needle-knife group 1 after treatment for 3 months was significantly lower than that of needle-knife group 2 (P < 0.05). After treatment for 2 weeks, the total effective rate of needle-knife group 1 was 93.3%, which of needle-knife group 2 was 90.0%. After treatment for 3 months, the total effective rate of needle-knife group 1 was 83.3%, which of needle-knife group 2 was 80.0%. There were no sinificant differences on the total effective rates between the two groups (P > 0.05). Conclusion Needle-knife treatment requires a certain course of treatment, and the effect is better on 2 weeks. Needle-knife is a reliable therapy for the treatment of cervicogenic headache. Its effectiveness and safety have been clinically confirmed and it is worthy of clinical promotion and application.

Research progress on needle-knife treatment of carpal tunnel syndrome

Carpal tunnel syndrome is a neurological disorder caused by the compression of the median nerve in the wrist. The anatomical structure of the carpal tunnel makes it easy for the transverse carpal ligament to thicken and compress the median nerve to trigger carpal tunnel syndrome. For mild to moderate carpal tunnel syndrome, needle knife treatment is more advantageous than other treatments. The needle knife treats the carpal tunnel syndrome by releasing the transverse wrist ligament to release carpal tunnel pressure. This article mainly discusses the related research progress from the aspects of the carpal tunnel anatomy, the treatment of the needle knife and the mechanism of needle knife treatment in recent years.

Research and thinking on the health work of adolescents in China

Adolescents (10-19 years old) are a key stage in the development of life. The health and development of adolescents will affect the health and development of their lives and even the next generation. In recent years, the health work of adolescents in China has been carried out mainly from four aspects: injury, nutrition, mental health and sexual and reproductive health. It has achieved certain results, mainly in the improvement of adolescents′ nutritional status and physical development indicators. But the mental health status of adolescents is generally poor, the lack of reproductive health knowledge reserves, the incidence of female abortions are increasing year by year, and the problem of overweight and poor eyesight among young people is more prominent. The problems of adolescent health in China are inseparable from China′s talent evaluation mechanism, public opinion propaganda and traditional culture. To promote the health and development of Chinese adolescents, we must improve the top-level design, create a good public opinion environment, and provide health education and health services that is compatible with the characteristics of adolescents.

Study on the compatible stability of Omeprazole Sodium for injection with 20 kinds of common TCM injections

Objective To study the stability of Omeprazole Sodium for Injection combined with 20 commonly used TCM injections in clinic, in order to provide basis for reasonable clinical use of Omeprazole Sodium. Methods Omeprazole Sodium for Injection was prepared at horizontal laminar flow in pharmacy intravenous admixture services (PIVAS) of Lanzhou General Hospital of PLA according to the doctor′s advice, and mixed with 20 TCM Injections at equal proportion. The appearance of the mixture was observed and the pH value and insoluble particles were detected in 0-6 h. Results The color changes appeared when Omeprazole Sodium mixed with Ginkgo Leaf Extract and Dipyridamole Injection, Extract of Folium Ginkgo Injection, Shuganning Injection, Safflor Yellow Injection, Breviscapine Injection, Salvia Miltiorrhiza and Ligustrazine Injection, Salvianolate Injection; the pH values were significantly reduced and the insoluble particles contents were increased obviously when Omeprazole Sodium mixed with 20 TCM injections (P < 0.05). Conclusion Omeprazole Sodium Injection should be avoided in compatibility with TCM injections, and the pipe should be flushed in sequential instillation.

Investigation and analysis on off-label drug use in Pediatric Outpatient of General Hospital of Ningxia Medical University in 2017

Objective To investigate and analyze the situation on off-label drug use for children in Pediatric Outpatient of General Hospital of Ningxia Medical University ("our hospital" for short), in order to provide a reasonable guidence for children medication in clinic. Methods From January to December 2017, Pediatric Outpatient drug prescriptions were selected randomly in our hospital. According to the medicine dispensatory, whether medication orders were over the package insert, the types and medication situation at different ages of off-label drug use, and the correlation between off-label drug use and basic medicine were analyzed. Results A total of 2640 medical prescriptions were selected from the Outpatient Department of Pediatrics, 4293 prescription medication advices were evaluated. According to the prescription and medication orders, the incidence rate of off-label drug use was 11.14% and 7.29% respectively. The main types of off-label drug use were the dosage and frequency of drug delivery, the incidence rates were 4.52% and 1.51% respectively, and the constituent ratios were 61.98% and 20.77% respectively, followed by the indications, the incidence of which was 0.96%, and the constituent ratio was 13.10%. Few references were related with children's medication information and contraindications. According to medication orders, the incidence rate of off-label drug use of the first two patient ages were neonatal (100.00%) and pre-school age (11.24%). The top four drugs of off-label drug use were dermatological drugs (20.00%), vitamin (16.93%), liver protection (13.64%) and digestive system and metabolic drugs (7.96%). Conclusion Due to the phenomenon of off-label drug use in Pediatric Outpatient in our hospital, the clinical pharmacists should pay close attention to the phenomenon and intervene rationally.

Ultrasonography observation and evaluation of sciatic neuropathy in type 2 diabetes mellitus

Objective To investigate ultrasonography characteristics of type 2 diabetes mellitus with sciatic neuropathy. Methods From February 2017 to February 2018, in the First People's Hospital of Neijiang City, 70 cases of diabetic patients with sciatic neuropathy (group A), 70 cases of diabetic patients without sciatic neuropathy (group B) were selected, and 70 healthy persons were selected as control (group C) at the same time. The characteristics of sciatic nerve in each group were observed by high frequency ultrasound. Results The proportion of nerve capsule thickening and rough in group A was significantly higher than those in group B and group C (P < 0.05); the proportion of thickening and rough of the perineal membrane, the reduction of the echo of the inner were compared among three groups, and the differences were not statistically significant (P > 0.05). The anteroposterior diameter (D1), transverse diameter (D2) and cross sectional area (CSA) of sciatic nerve in the group A were significantly higher than those in the group B and group C (P < 0.05); the D1, D2 and CSA of sciatic nerve in group B and group C were compared, the differences were not statistically significant (P > 0.05). In group A, the D1, D2 and CSA of male patients were significantly higher than those of female patients (P < 0.05). Conclusion High frequency ultrasound can clearly display the parameters of sciatic nerve, and it can provide objective evidence for the diagnosis and treatment of type 2 diabetic sciatic neuropathy.

Influence factors of student nurses′ workplace satisfaction during their initial stage of clinical practice and correlation study of clinical belongingness

Objective To survey the undergraduate and junior college student nurses′ satisfaction of workplace during their initial stage of clinical practice, analyze factors that may influence student nurses′ workplace satisfaction, and discuss the relationship between it and clinical belonging. Methods From 20th September to 20 th October 2017, 128 student nurses practicing for 2-3 months in 2 grade-A tertiary hospitals in Beijing were surveyed, with the general information, nursing workplace satisfaction questionnaire (NWSQ) and belongingness scale: clinical placement experience (BES:CPE). Results Among 128 student nurses, total points of job satisfaction scores was (66.77±14.09) scores, total points of clinical belonging score was (125.05±20.03) scores, there were significant differences between workplace satisfaction of student nurses with different gender, education, place of origin, individual voluntary and career planning (P < 0.05), and it was positive correlation with clinical belonging (r = 0.67, P < 0.05). Conclusion Nursing students′ gender, education, place of origin, individual voluntary ,career planning and sense of belonging influenced their workplace satisfaction during their initial stage of clinical practice. Workplace satisfaction is a key determinant of career decisions and influence student nurses′ decisions to continue in the profession.
