SONG Kaiyong1 XIONG Weijian1 TIAN Lu1 XIONG Mengyuan2 TAO Wenqiang1 WANG Miao1
DOI: 10.20047/j.issn1673-7210.2023.34.07
More than 10% of the population is affected by abnormal kidney function, and most kidney diseases are characterized by a breakdown of the glomerular filtration barrier. Glomerular filtration barrier is a unique multilayered structure composed of glomerular endothelial cells, basement membrane, and podocytes. When lipid metabolism is disturbed, it will induce P2X7R-NLRP3 inflammatory body axis, sterol-regulatory element binding protein, vascular endothelial growth factor-B, junction adhesion molecule-like protein, sterol-O-acyl transferase 1, ATP-binding cassette transporter A1, and other proteases and factors to produce inflammatory response and oxidative stress. This paper focuses on the effects of lipid toxicity caused by ectopic lipid deposition on glomerular filtration barrier, which will provide a new idea for the treatment of diabetic kidney disease.

GAO Chang MA Liang GOU Shenju▲
DOI: 10.20047/j.issn1673-7210.2023.34.09
Under the current treatment strategy of glucocorticoid combined with immunosuppressants, infection has become one of the main complications and early causes of death in the treatment of anti-neutrophil cytoplasmic antibody associated vasculitis (AAV). At the same time, infection can also induce and aggravate AAV through various mechanisms, increasing the incidence and recurrence rate of AAV. In clinical practice, the clinical manifestations of active AAV and infection are similar, and it is easy to be misdiagnosed. It is necessary to distinguish infection and AAV from multiple levels and use multiple detection methods to achieve early diagnosis and treatment. With the continuous improvement of current detection methods and in-depth understanding of pathogens, it will be beneficial to further understand the close relationship between infection and AAV, and provide help for clinical differential diagnosis and treatment of the two.

53 Current status of research on treatment of knee osteoarthritis with platelet-rich plasma based on Wnt/β-catenin signaling pathway
DENG Lei1 WANG Shaohua2 HOU Yingzhou2 YANG Guangya1 LI Kewei2 SUN Bo2
DOI: 10.20047/j.issn1673-7210.2023.34.11
Knee osteoarthritis (KOA) is a chronic degenerative bone disease characterized by the wear of articular cartilage. In the early stage, the main manifestations are knee pain, swelling, and limited joint movement, which can eventually lead to locking of the knee, deformity, and disability. Wnt/β-catenin signaling pathway plays an important role in the pathogenesis and development of KOA, which influences the metabolic balance of articular cartilage and subchondral bone by regulating cell proliferation and differentiation. Platelet-rich plasma (PRP), as a new treatment for KOA, can slow down the pathologic progression of KOA by delaying cartilage degeneration, promoting cartilage regeneration, and inhibiting intra-articular inflammation, which may be related to its involvement in the regulation of Wnt/β-catenin signaling pathway. This article mainly introduces the pathological relationship between Wnt/β-catenin signaling pathway and KOA, the application of PRP in the treatment of KOA, and the research status of PRP based on Wnt/β-catenin pathway in the treatment of KOA, aiming to provide more theoretical basis for the clinical treatment of KOA with PRP.

DOI: 10.20047/j.issn1673-7210.2023.34.12
With the increasing pressure of life, the prevalence of non-puerperal mastitis in women is increasing. How to effectively diagnose and treat this disease situation is urgent. At present, clinical experience has confirmed that traditional Chinese medicine is the most effective way to treat non-puerperal mastitis. However, Chinese medicine and western medicine are two relatively independent diagnosis and treatment systems, and the theory of traditional Chinese medicine is mostly ancient traditional philosophical thought, which is different from the modernization of western medicine, resulting in the inability to popularize the diagnosis and treatment of traditional Chinese medicine in western medicine hospitals. Therefore, if traditional Chinese medicine factors are applied to the diagnosis and treatment of western medicine systems for multi-omics combination, it is expected to solve this problem. As one of the objective indicators in traditional Chinese medicine theory, traditional Chinese medicine tongue elephant is relatively intuitive and clear, and is expected to become one of the factors in the diagnosis of non-puerperal mastitis by combining Chinese medicine and western medicine. This article mainly discusses the research progress of clinical symptomatology, ultrasound imaging, and Chinese medicine tongue imaging in non-puerperal mastitis, and combines the development process of artificial intelligence disease diagnosis to form a precision system of integrated traditional Chinese and western medicine for multiomics joint diagnosis of this disease, which is expected to promote the homogeneous promotion of this technology in western medicine hospitals and grassroots hospitals.

Results The BBT values of the left hand and the right hand were (38±8) number/min and (39±8) number/min respectively. The grip strength of the dominant hand was (166.8±75.0) N. The correlation results showed that BBT left hand test was positively correlated with BBT right hand test, grip strength, Schober test, 30 s sitting and standing test, functional extension test, right back scratching test, left back scratching test, static balance and coordination function, and was negatively correlated with age and timing standing and walking test (P<0.05). BBT right hand test was positively correlated with grip strength, Schober test, 30 s sitting and standing test, functional extension test, right back scratching test, left back scratching test, static balance and coordination function, but was negatively correlated with age and timing standing and walking test (P<0.05). Grip strength was positively correlated with 6 min walking test, Schober test, 30 s sitting and standing test, functional stretching test, static balance and coordination function, and was negatively correlated with timed standing and walking test (P<0.05). 6min walking test, timed standing up and walking test, and functional stretching test were all influencing factors of grip strength, among which the test value of timed standing up and walking test has the greatest influence on grip strength (B=-14.916, P=0.030). The regression equation was as follows: grip strength =73.31+0.26×6 min walking test -14.91× timed stand up and walk test +3.61× functional stretching test. Factors affecting hand flexibility included 30 s sitting and standing test, functional extension test, and Schober test, among which Schober test value (B=1.140, P=0.029) had the greatest influence on hand flexibility. Regression equation: hand flexibility =2.58+0.70×30 s sitting and standing +0.25× functional stretching test +1.14×Schober test. Conclusion The manual flexibility and grip strength of the elderly are correlated with functional tasks that reflect postural control, such as standing up and walking, sitting and standing for 30 s, functional extension, static balance, coordination and lumbar motion, among which functional extension test is a common factor affecting grip strength and hand flexibility. It is speculated that hand function assessment may be one of the important methods to indirectly reflect functional extension test.

Objective To investigate the distribution of traditional Chinese medicine syndromes of upper airway cough syndrome. Methods A total of 117 patients with upper airway cough syndrome admitted to the Department of Respiratory Heat Disease, Dongfang Hospital of Beijing University of Chinese Medicine from January 2009 to August 2023 were selected as the study objects. Their gender, age, medical history, and syndromes were statistically analyzed, and the high-frequency symptoms and commonly used Chinese medicines were systematically clustered. Results A total of 159 times were diagnosed in 117 patients, among which the syndrome of wind pathogen invading lung was the most, followed by the syndrome of dampness-heat stagnated in lung-defense phase and syndrome of phlegm-damp amassing in lung. The traditional Chinese medicine syndrome type of patients with upper airway cough syndrome was related to age (P<0.05), but was not related to sex or common rhinitis disease type (P>0.05). Traditional Chinese medicine syndromes were characterized by lung symptoms, wind, phlegm, dampness, and heat. Systematic clustering could be divided into three association combinations, including the syndrome of wind pathogen invading lung, the syndrome of dampness-heat stagnated in lung-defense phase, and syndrome of phlegm-damp amassing in lung. By systematic clustering, the commonly used Chinese medicine can be divided into three groups: the drugs of activating qi-flowing and dissipating phlegm for resuscitation, clearing heat and eliminating dampness, and dispelling wind and arresting cough. Conclusion The main syndromes of upper airway cough syndrome are wind evil and lung stagnation, followed by dampness-heat stagnation and phlegm-dampness accumulation of lung. The manifestations of wind, phlegm, dampness, and heat are common. Therefore, targeted treatment can be carried out in clinic.

154 Professor Li Junxiang’s experience in treating chronic recurrent ulcerative colitis with New Wumei Pills
DENG Yuqing LI Junxiang MAO Tangyou WANG Muyuan CHEN Xiaowei
DOI: 10.20047/j.issn1673-7210.2023.34.33
Chronic recurrent ulcerative colitis is a commonly encountered subtype of ulcerative colitis within clinical practice, known for its persistent and refractory characteristics. Professor Li Junxiang from Dongfang Hospital, Beijing University of Chinese Medicine emphasizes that the underlying cause of chronic recurrent ulcerative colitis resides in the insufficiency of both the spleen and kidney, giving rise to symptoms of dampness and heat obstruction, as well as qi-stagnation and blood stasis. Primary therapeutic strategies encompass the elimination of dampness and heat, facilitation of blood circulation to alleviate blood stasis, and fortification of kidney and spleen functions. Drawing upon extensive clinical experience and a comprehensive assimilation of medical knowledge across various historical periods, Professor Li Junxiang has refined the traditional Wumei Pills Prescription from the Treatise on Cold Pathogenic Diseases by incorporating personalized adjustments. This innovative approach, known as the New Wumei Pills Prescription, has demonstrated remarkable efficacy in clinical practice, yielding favorable treatment outcomes. This paper mainly encapsulates the theoretical underpinnings and facilitates a clinical discourse concerning the treatment of chronic recurrent ulcerative colitis with the New Wumei Pills Prescription. Furthermore, a validated case is appended to furnish empirical substantiation.

DOI: 10.20047/j.issn1673-7210.2023.34.35
Professor Wang Lie, the master of traditional Chinese medicine, treats children with chronic eczema, according to the characteristics of pathogenesis, the disease is divided into attack stage, remission stage, and stable stage. Professor Wang believes that in the attack period, excluding the pathogenic wind, damp, and heat of exogenous disease, there are still five zang viscera consumed injury, mainly the liver, spleen, and lung. Therefore, we should further clear heat and dissipate dampness, desensitizing and dispelling wind, and Jiajian Tuomin Powder should be given. In the remission stage, the heart and liver are mainly consumed, the treatment is to nourishing blood for tranquillization, supplemented by regulating and tonifying the spleen and kidney, Jiajian Anshen Decoction is commonly used. In the stable stage, the main symptoms are insufficient nephros and lung yin depletion injury. The treatment was to nourishing the lung and kidney, supplementing qi and tonifying blood, and Jiajian Buxue Powder should be given. In the treatment, special attention should be paid to the protection after recovery, and the sequential treatment has a lasting effect.

DOI: 10.20047/j.issn1673-7210.2023.34.36
Bladder detrusor underactivity refers to a series of lower urinary tract symptoms caused by the delayed or incomplete emptying of the bladder during the normal urination period due to the reduced contractility or duration of the bladder detrusor. The serious complications include upper urinary tract damage and renal failure, which threaten the life and health of patients. At present, the treatment methods at home and abroad are limited, and there is still a lack of targeted treatment programs. Through many years of clinical observation and practical experience, Professor Lu Jianxin believes that the key pathogenesis of this disease lies in “qi mechanism disorder”, and puts forward the treatment idea of “regulating qi and restoring”. At the same time, Professor Lu Jianxin also found that such patients are often accompanied by “relative obstruction” problems, which makes the disease further aggravated. Therefore, Professor Lu Jianxin through the integration of traditional Chinese and western medicine treatment, on the basis of the operation to remove the “relative obstruction”, combined with the traditional Chinese medicine thought of “conditioning qi mechanism” throughout the whole, syndrome differentiation and subtraction. This also reflects Professor Lu Jianxin’s medical philosophy of “accepting the strengths of each institution, integrating Chinese and western medicine, and striving for effectiveness”.

DOI: 10.20047/j.issn1673-7210.2023.34.37
Tinnitus is a common disease and complication in clinic, which is more common in adults. In recent years, with the change of lifestyle and the increase of learning pressure, more and more children have been diagnosed with tinnitus, especially for children who do not correctly express the condition, which often leads to delay of the condition and brings difficulties to clinical diagnosis and treatment. Therefore, tinnitus should be paid attention to in pediatrics. This paper summarizes the clinical experience of Professor Wang Youpeng, a famous traditional Chinese medicine doctor, in the treatment of tinnitus, and concludes that characteristics of children in cold areas are prone to external evil, dampness and heat in the spleen and stomach, and fire of the heart and liver. The etiology and pathogenesis are divided into four aspects: wind-cold external attack, phlegm turbidness and dampness, liver-qi stagnation, and kidney unit loss. In the treatment, attention should be paid to both manifestation and root cause, strengthening vital qi to eliminate pathogenic factor, so as to stop tinnitus and clear the ear. At the same time, regulate children’s psychological problems in order to treat the mind and body together. Attached to the document is a detailed explanation of the case.
