Abstract  Objective To explore the action mechanism of Bizhengning Decoction against wind-cold-dampness bi type rheumatoid arthritis based on the method of serum metabolomics. Methods A total of 24 SPF Wistar female rats, 160-180 g at seven weeks were divided into normal group, model group, and Bizhengning Decoction group according to the random number table method, with eight rats in each group. Wind-cold-dampness bi type rheumatoid arthritis rat model was prepared in model group and Bizhengning Decoction group, after seven days of modeling, there was limited movement, obvious swelling of the right foot, swelling of the left joint, and deformation of the relevant joint, indicating successful modeling. Bizhengning Decoction group was given 17.86 g/(kg·d) intervention for 28 days, and normal group and model group were given the same amount of normal saline for 28 days. The changes of hair color, diet, walking activity, and other general conditions were observed; differential metabolites and metabolic pathways were analyzed and screened. Results Compared with normal group, rats in the model group had weak mental state, less glossy hair, less dietary water intake, and obvious lameness on the right foot when walking; compared with model group, the mental state, hair luster, eating water, and walking lameness of right foot were improved in Bizhengning Decoction group. Compared with the normal group, the synovial tissue was hyperplasia, the joint cavity was blurred and narrow, and the inflammatory cell infiltration was obvious, the joint bone was destroyed, the joint surface was rough and uneven, and pannus was formed in the rate of the model group; compared with model group, the synovial tissue proliferation decreased, the joint cavity became clear and enlarged gradually, the inflammatory cell infiltration decreased significantly, the joint bone destruction and the joint surface roughness improved significantly, and the number of pannus decreased significantly in Bizhengning Decoction group. There were 45 different metabolites in normal group and model group, and 51 different metabolites in model group and Bizhengning Decoction group; the main pathways involved steroid hormone biosynthesis, arginine and proline metabolism, etc. Conclusion Bizhengning Decoction may affects the metabolism of arginine and proline, porphyrin and chlorophyll, and glycine, serine and threonine, etc.
