4 Study on pharmacoynamics and safety of Shenbai-Qudu Teabag

Objective To observe the pharmacodynamics and safety indicators of Shenbai-Qudu Teabag. Methods A total of 60 SD rats (30 females and 30 males) with body weight of 150-170 g were divided into blank group, Shenbai high (1.050 g crude drug/ml), medium (0.525 g crude drug/ml), and low (0.263 g crude drug/ml) dose groups and Piyanping (0.600 g/ml) group according to random number table method, with 12 rats in each group. 0.1 ml of 1% carrageenan was injected subcutaneously into the left posterior foot of rats, the hourly plantar volume was measured between 1-5 h after administration, and the swelling degree was calculated. Forty-eight Kunming mice, females, weight 18-24 g, were divided into blank group, Shenbai high (1.750 g crude drug/ml), medium dose(0.875 g crude drug/ml), and low dose (0.438 g crude drug/ml) groups according to random number table method, with 12 cases in each group. The basal pain threshold of mice was determined, and then the mice were soaked in a drug bath at 40℃ for 20 minutes, respectively. The pain threshold of mice was measured at five time points after administration, and the percentage increase of pain threshold was calculated. A total of 30 male and female guinea pigs with body weight of 280-320 g were divided into blank group, 2, 4-dinitrochlorobenzene group, and ginseng and cypress group according to random number table method, with 10 guinea pigs in each group. 0.2 ml of Shenbai was taken and apply it to the right depilatory area for 6 h, and repeat once on the 7th and 14th day respectively. 14 days after the last administration, 0.2 ml of the drug was applied to the left depilatory area of guinea pigs, and the local skin and the whole body situation immediately and 24, 48, and 72 h for evaluation were observed. Results The swelling degree of foot in Shenbai high dose group and medium dose group at 5 h were lower than that at 3 h (P<0.05). The foot swelling of Shenbai high dose group and medium dose group was lower than that of blank group at 2-3 h (P<0.05), and the foot swelling of Shenbai low dose group was lower than that of blank group at 3 h (P<0.05). The basal pain threshold of Shenbai high dose group for 0.5-4.5 h, medium dose group for 1.5-4.5 h and low dose group for 3.5-4.5 h were higher than basic pain threshold of this group(P<0.05). The pain threshold of Shenbai high dose group was higher than that of blank group at 0.5 ~ 4.5 h (P<0.05), and that of medium dose group at 1.5-3.5 h was higher than that of blank group (P<0.05). Shenbai-Qudu Teabag had no sensitization effect. Conclusion Shenbai-Qudu Teabag has significant anti-inflammatory and analgesic effects when applied externally, and it is safe for topical use.

8 Visual analysis of research progress of wrist and ankle acupuncture based on VOSviewer and CiteSpace

Objective To explore the domestic and foreign literature in the field of wrist and ankle acupuncture based on CiteSpace and VOSviewer in the past 22 years were mined, and the knowledge graph was visualized and analyzed. Methods Through searching the databases of CNKI and Web of Science, the literature related to wrist and ankle acupuncture from January 2000 to April 2023 were collected, author distribution, research unit distribution, keyword co-occurrence, knowledge base, and research frontier were discussed. Results A total of 767 pieces of literatures were included, and the number of publications related to wrist and ankle acupuncture increased year by year from 2014. The domestic core research team was Zhou Qinghui’s team; the international core research teams were Fang, Fanfu’s team and Hao, Wei’s team. The core research unit was the Naval Medical University; domestic research focused on the intervention of wrist and ankle acupuncture in stroke, insomnia, postoperative analgesia, shoulder pain, and so on. International research focused on randomized controlled studies and mechanism studies. Conclusion The visualization map shows the research status of wrist and ankle acupuncture. The research team of Naval Medical University is the main force of wrist and ankle acupuncture research, and the issuing units should strengthen cross-regional cooperation. On the basis of existing studies, the mechanism of analgesia with wrist and ankle acupuncture is deeply discussed, and a high-quality randomized controlled study is carried out.

15 Expression of TIPE2, Ki-67 in endometrial cancer tissues and their relationship with pathological features

Objective To analyze the expression of TIPE2 and Ki-67 in the tissues of patients with endometrial cancer and their relationship with pathological features. Methods From January 2018 to January 2020, 60 patients with endometrial cancer treated in the Affiliated Hospital of Hebei Engineering University (hereinafter referred to as “our hospital”) were selected as case group, and another 60 patients with endometrial atypical hyperplasia treated in our hospital during the same period were selected as control group. The expression levels of TIPE2 and Ki-67 were compared between two groups; the expression levels of TIPE2 and Ki-67 in endometrial carcinoma patients with different pathological characteristics were compared; and the correlation of the expression of TIPE2 and Ki-67 with pathological features was analyzed. Results The negative rate of TIPE2 and the positive rate of Ki-67 in case group were higher than those in the control group, and the differences were statistically significant (P<0.05). The negative rate of TIPE2 and the positive rate of Ki-67 in patients with endometrial cancer were statistically significant in different Federation International of Gynecology and Obstetrics (FIGO) stages, histological grades, degree of muscular invasion, and lymph node metastasis (P<0.05); there were no significant differences in the negative rate of TIPE2 and the positive rate of Ki-67 with different pathological types and tumor diameters in patients with endometrial cancer (P>0.05). The expression of TIPE2 negative and Ki-67 positive were positively correlated with FIGO stage, histological grade, degree of muscle infiltration, and lymph node metastasis (rs>0, P<0.05). Conclusion The negative rate of TIPE2 and the positive rate of Ki-67 in patients with endometrial carcinoma is high, which can be used to evaluate the potential of invasion and metastasis, pathological stage, and histological grade of cancer cells.

20 Discussion on the medication rules of Director Shen Hongquan in the treatment of non-alcoholic fatty liver disease based on data mining

Objective To explore the medication rules of Director Shen Hongquan in the treatment of non-alcoholic fatty liver disease (NAFLD) based on the cloud platform of ancient and modern medical records. Methods A total of 143 patients with NAFLD treated by Director Shen Hongquan in the Outpatient Department of Putuo Hospital, Shanghai University of Traditional Chinese Medicine from December 2021 to December 2022 were selected as the study objects. Effective medical record data were collected and entered, and after standardized processing through the cloud platform of ancient and modern medical records, data mining was carried out on indicators such as drug frequency, nature, flavour, channel tropism distribution and correlation, cluster analysis, and complex network analysis, so as to summarize the medication rules of Director Shen Hongquan. Results A total of 143 prescriptions covering 300 kinds of traditional Chinese medicine. The drug nature were mainly calm, slightly cold, and warm. The drug flavours were bitter and sweet. The channel tropism were mainly liver channel and spleen channels. Crataegi Fructus, Nelumbinis Folium, Cassiae Semen, and so on were high-frequency drugs. The data of 18 association rules were obtained by association analysis of traditional Chinese medicine compatibility. Cluster analysis separated four groups of drugs, and complex network analysis identified 15 core drugs. Conclusion Director Shen Hongquan treated NAFLD with the principle of “soothing and softening liver, invigorating spleen for resolving turbidity”, reinforcement and elimination in combination, treating both the tip and root.

25 Effects of acupuncture on the content of hydrogen sulfide and the expression of cystathionine-β-synthase and cystathione-γ lyase in the lungs of rats with chronic obstructive pulmonary disease

Objective To observe the effects of acupuncture on the content of hydrogen sulfide (H2S) and the expression of cystathionine-β-synthase (CBS) and cystathionine-γ lyase (CSE) in lung tissue of chronic obstructive pulmonary disease (COPD) rats. Methods A total of 24 6-week-old clean grade male SD rats with body weight of (200±10) g were selected, and divided into control group, model group, and electroacupuncture group by random number table method, with eight rats in each group. The control group was fed conventionally. The COPD model was established by cigarette smoke combined with lipopolysaccharide in model group and electroacupuncture group. After successful modeling, the electroacupuncture group was treated with electricacupuncture on both fèishù and zúsānlǐ for two consecutive weeks, while the model group and control group were not treated. After the intervention, forced vital capacity (FVC), forced expiratory volume in 0.1 second (FEV0.1), FEV0.3, and peak expiratory flow (PEF) were measured. The pathological changes in the lungs of the three groups were observed. The levels of interleukin-1β (IL-1β) and tumor necrosis factor-α (TNF-α) in lung tissue and plasma of the three groups were detected by enzyme-linked immunosorbent assay. The contents of H2S, CBS, and CSE in lung tissue were detected. The correlation between IL-1β and TNF-α in lung tissue and H2S was further analyzed. Results FEV0.1, FEV0.3, FEV0.1/FVC, FEV0.3/FVC, and PEF in the model group were lower than those in the control group, and the differences were statistically significant (P<0.05). Its pulmonary pathological results showed inflammatory cell infiltration and pulmonary interstitial thickening. IL-1β and TNF-α in lung tissue and plasma in model group were higher than those in control group, while H2S, CBS, CSE contents, and CBS, CSE protein expressions in lung tissue were higher than those in control group, and the differences were highly statistically significant (P<0.01). FEV0.1, FEV0.3, FEV0.1/FVC, FEV0.3/FVC, and PEF in electroacupuncture group were higher than those in model group, and the differences were statistically significant (P<0.05). IL-1β and TNF-α in lung tissue and plasma in the electroacupuncture group were lower than those in the model group, while the H2S, CBS, CSE contents and the CBS and CSE protein expression in lung tissue were lower than those in the model group, the differences were statistically significant (P<0.05). Correlation analysis showed that IL-1β, TNF-α, and H2S in lung tissue were positively correlated (r>0, P<0.01). Conclusion Electroacupuncture in fèishù and zúsānlǐ can down-regulate the content and expression of H2S, CSE, and CBS in lung tissue of COPD rats, and H2S may be involved in the anti-infection effect of acupuncture.

29 Application analysis of ultrasonic, CT features and combined diagnosis in papillary thyroid carcinoma cervical lymph node metastasis

Objective To investigate the diagnostic value of ultrasonography, CT features and combined application of cervical lymph node metastasis in papillary thyroid carcinoma (PTC). Methods A total of 139 PTC patients diagnosed in the Affiliated Hospital of Guizhou Medical University from January 2019 to December 2021 were selected and divided into 66 patients with cervical lymph node metastasis (metastatic group) and 73 patients with no cervical lymph node metastasis (non-metastatic group) according to the results of cervical lymph node puncture. Preoperative ultrasound and CT imaging data of the two groups were retrospectively analyzed, the imaging features of PTC lesions and metastatic lymph nodes were compared between the two groups, and the value of the two methods in the diagnosis of cervical lymph node metastasis of PTC was calculated. Results The difference between the two groups was statistically significant (P<0.05) in the ratio of lymph node margin defect, vertical and horizontal diameter, portal echo, lymph node shape, and echo feature ratio. There were significant differences between the two groups in lymph node internal capsular necrosis, granular calcification, lymph node margin enhancement, lymph node morphology, lymph node and surrounding tissue, and lymph node elongated diameter line ratio(P<0.05). Conclusion After cervical lymph node metastasis of PTC, ultrasound, and CT have obvious characteristics, and the combined application of the two can improve the diagnostic efficiency.

34 Research progress on the mechanism of autophagy in cardiovascular disease and its risk factors

Autophagy is a mechanism of cellular self-protection that plays an important role in cellular homeostasis. Autophagy is closely related to cardiovascular diseases. It affects the occurrence and development of myocardial ischemia-reperfusion injury, cardiomyopathy, coronary atherosclerosis, arrhythmia, heart failure, etc. by regulating the expression of autophagy related genes, mitochondrial function, oxidative stress, inflammation, Ca2+ level, and lipid accumulation. Moreover, autophagy can also affect the risk factors of cardiovascular disease by regulating levels of blood sugar, blood pressure, cholesterol, and uric acid. This article will review the mechanisms by which autophagy plays a role in cardiovascular disease and its risk factors, providing new directions for the development of cardiovascular disease prevention and treatment drugs and the selection of potential targets.

39 Current research status of artificial intelligence for endometrial cancer recognition in nuclear magnetic images

With the increasing incidence rate of endometrial cancer, accurate and rapid early diagnosis is crucial for the treatment and prognosis of patients. Magnetic resonance imaging plays an important role in the diagnosis of endometrial cancer, however, relying on traditional visual interpretation still has some limitations. In recent years, the rapid development of artificial intelligence technology has brought new possibilities for medical imaging diagnosis. Through deep learning and machine learning algorithms, features can be effectively extracted from a large amount of magnetic resonance imaging data, and endometrial cancer lesions can be automatically identified, further assisting doctors in disease staging and treatment decision-making. At present, the application of artificial intelligence in endometrial cancer is still in the exploratory stage, and it still faces some challenges. The scale and diversity of datasets are key issues in current research. In addition, applying these technologies to clinical practice requires more clinical validation and strict regulation. Overall, the use of artificial intelligence for identifying magnetic resonance images further demonstrates potential application value. With the further development of technology and the continuous accumulation of more data, it is believed that artificial intelligence will play an increasingly important role in the early diagnosis and treatment of endometrial cancer.

43 Research progress on the biological role of CD155 and its application in bladder cancer

CD155 is a glycoprotein in the immunoglobulin superfamily, which is involved in cell motility, signaling, and T-cell-mediated immunity. It can mediate oncologic immunity by interacting with ligands(TIGIT, CD96, and CD226) to regulate the function of tumor infiltrating lymphocytes. CD155 is up-regulated in bladder cancer and plays an important regulatory role in the invasion of bladder cancer, which is associated with poor prognosis of patients. Immunotherapy that blocks CD155 signal is expected to improve the prognosis of patients with bladder cancer. In this paper, the biological effects of CD155 and its application in bladder cancer are reviewed from the aspects of immune regulation of CD155, the diagnostic value for bladder cancer, and the relationship with the prognosis of patients with bladder cancer and related immunotherapy, in order to provide new ideas for the diagnosis and treatment of bladder cancer.

47 Study on job satisfaction of medical staff in a hospital in Beijing

Objective To analyze the current situation and influencing factors of medical staff’s job satisfaction in a hospital in Beijing, in order to improve the job satisfaction of medical staff. Methods In January 2021, a questionnaire was used to collect job satisfaction data of 450 on-the-job medical staff in the hospital in Beijing from five dimensions: job itself, job return, internal environment, practice environment, and organization management, and to compare the job satisfaction of medical staff with different indicators. Results A total of 450 questionnaires were distributed and 397 were effectively collected, with an effective recovery rate of 93.85%. According to the data analysis, the medical staff in this hospital had the highest score of satisfaction with the working environment and the lowest score of satisfaction with the work return. There were statistically significant differences in satisfaction scores of medical staff of different ages in work itself, internal environment, practice environment, and organization and management (P<0.05). There were statistically significant differences in satisfaction scores of different marital status, professional title, job nature, average monthly income, and working environment of medical staff (P<0.05). There were significant differences in job satisfaction scores among medical staff with different educational backgrounds and administrative positions (P<0.05). There were statistically significant differences in satisfaction scores of medical staff with different working years in work itself, work return, practice environment, and organization and management (P<0.05). Conclusion The overall job satisfaction of medical staff in this hospital is at a high level. The job satisfaction of medical staff can be further improved by improving the salary distribution system, optimizing the internal and organizational environment, and differentiated management.

51 Influencing factors of cognitive disorder in elderly patients with atrial fibrillation who take oral Warfarin

Objective To investigate the influencing factors of cognitive disorder (CD) in elderly patients with oral Warfarin atrial fibrillation (AF). Methods A total of 121 patients with atrial fibrillation admitted to Chengde Central Hospital, Hebei Province from February 2019 to February 2022 were selected in a cross-sectional survey. The patients were divided into group without CD (59 cases) and group with CD (62 cases) by using the cognitive disorder rating scale (CDR). The drug use, medical history, and demographic data were collected. Logistic regression analysis was used to analyze the influencing factors of cognitive impairment, and receiver operating characteristic (ROC) curve was plotted to evaluate the predictive value of oral Warfarin in CD. Results There were significant differences in Warfarin anticoagulant stability, hypertension, diabetes, and left ventricular ejection fraction (LVEF) among patients with different CD patients (P<0.05). Logistic regression analysis showed that hypertension (OR [95%CI] =3.003 [1.184, 7.617]), diabetes (OR [95%CI] =4.347 [1.756, 10.759]), Warfarin anticoagulation (OR [95%CI] =3.915 [1.572, 9.750]), and LVEF (OR [95%CI] =0.893 [0.826, 0.966]) were the influencing factors for CD in elderly patients with AF(P<0.05). The area under the curve (AUC [95%CI]) of Warfarin anticoagulant treatment was 0.802 (0.725-0.879), and the sensitivity and specificity of CD were 83.9% and 62.7% when the maximum Jorden index was 0.466. Conclusion Diabetes, hypertension, warfarin anticoagulant treatment stability, LVEF are influencing factors for CD in patients with AF, Warfarin anticoagulant treatment stability has a good predictive value for CD in patients with AF.

56 Relationship between self-acceptance and family closeness and adaptability in patients with cervical cancer after total hysterectomy

Objective To analyze the relationship between self-acceptance and family closeness and adaptability in patients with cervical cancer after total hysterectomy. Methods A total of 100 patients with cervical cancer who underwent total hysterectomy in the No.900 Hospital of Joint Logistics Support Force of the Chinese People’s Liberation Army from January 2021 to August 2022 were selected as study object by convenience sampling method. Self-acceptance questionnaire (SAQ) and the Chinese version of family closeness and adaptability scale (FACES Ⅱ-CV) were used for questionnaire investigation. The correlation between self-acceptance and family closeness and adaptability of cervical cancer patients after total hysterectomy was analyzed. Results The total score of self-acceptance was (49.77±3.06) points, and the total score of family closeness and adaptability was (112.17±12.47) points. There were significant differences in SAQ and FACES-Ⅱ scores among patients with different education level, clinical stage, monthly income, types of underlying diseases, and places of residence (P<0.05). Pearson correlation showed that SAQ was positively correlated with FACES-Ⅱ after total hysterectomy (r>0, P<0.05). Conclusion There is a positive correlation between self-acceptance and family intimacy and adaptability in patients with cervical cancer after total hysterectomy, that is, the higher the level of self-acceptance, the higher the level of family intimacy and adaptability.

60 Prediction of spontaneous circulatory recovery in patients with cardiopulmonary resuscitation after out-of-hospital cardiac arrest based on machine learning algorithm

Objective To predict the recovery of spontaneous circulation in patients with out-of-hospital cardiac arrest and cardiopulmonary resuscitation (CPR) by machine learning algorithm, so as to provide data support for improving the success rate of CPR in patients with out-of-hospital cardiac arrest. Methods From the medical records of 1 201 patients with cardiac arrest and CPR in the emergency patient database (EPD) of Hebei Provincial Emergency Technology Innovation Center from September 2018 to April 2022, 463 patients with out-of-hospital cardiac arrest were screened out. The data were preprocessed with python language pandas and combined with clinical characteristics. The 14 variables with the highest accuracy were determined. 75% of the samples were divided into training sets to build a model, and four machine learning algorithms including logistic regression, parameter interpretation, gradient lifting, and random forest were used to train and test the data. 25% of the samples were divided into test sets for verification. The model performance was evaluated according to accuracy, accuracy, recall, receiver characteristic operating characteristic curve (ROC), etc., and the appropriate model was selected for influencing factor analysis. Results There were statistically significant differences in age, CPR start time, history of electrolyte disturbance, witness of cardiac arrest, CPR, defibrillation, and airway management using simple respirator between the return of spontaneous circulation(ROSC) group(57 cases) and failed group(406 cases) (P<0.05). The area under the curve (AUC) values of logistic regression, random forest, parameter interpretation, and gradient lifting were 0.73, 0.87, 0.90, and 0.86, respectively. The random forest model (accuracy=0.89, accuracy=0.90, recall=0.89, AUC=0.87) was selected to calculate the importance of each characteristic value. The results showed that the main factors affecting the recovery of spontaneous circulation in patients with out-of-hospital cardiac arrest were age, start time of CPR, previous history, witnessing cardiac arrest, giving external chest compression, and electrical defibrillation. Conclusion Machine learning has the potential to predict spontaneous circulatory recovery in patients with out-of-hospital cardiac arrest and CPR, and the random forest model works best.

Objective To explore the predictive value of CHA2DS2-VASc score combined with serum homocysteine (Hcy) and D-dimer (D-D) in the risk of thromboembolism in patients with nonvalvular atrial fibrillation (NVAF). Methods The clinical data of 102 patients with NVAF admitted to the 904th Hospital of the Joint Logistics Support Force of the Chinese People’s Liberation Army from June 2019 to October 2021 were retrospectively analyzed. Among them, 31 patients with thromboembolism were selected as the thrombus group, and 71 patients without thromboembolism were selected as the non-thrombotic group. Clinical data, CHA2DS2-VASc score, and serum Hcy, D-D levels were compared between the two groups. The influencing factors of thromboembolism in NVAF patients were analyzed, and receiver operating characteristics (ROC) curves were prepared. Area under the curve (AUC) was used to analyze the predictive value of CHA2DS2-VASc score, and serum Hcy and D-D for thromboembolism in patients with NVAF. Results The proportion of persistent/permanent atrial fibrillation, anteroposterior diameter of left atrium, left ventricular end-diastolic diameter, white blood cell count, uric acid, interleukin-1β, C-reactive protein, fibrinogen, CHA2DS2-VASc score, and serum Hcy, D-D levels in thrombus group were higher than those in non-thrombotic group, and left ventricular ejection fraction was lower than that in non-thrombotic group (P<0.05). The results of multivariate analysis showed that CHA2DS2-VASc score (OR=3.177, 95%CI: 1.326-7.613), serum Hcy (OR=2.869, 95%CI: 1.197-6.875), serum D-D (OR= 3.347, 95%CI: 1.397-8.019) were the influencing factors of thromboembolism in NVAF patients (P<0.05). ROC analysis showed that the AUC predicted by the combination of CHA2DS2-VASc score, serum Hcy and D-D was higher than the AUC predicted by the combination of CHA2DS2-VASc score and serum Hcy and D-D in NVAF patients (P<0.05). Conclusion The combination of CHA2DS2-VASc score, and serum Hcy, D-D levels has a high predictive effect on the risk of thromboembolism in patients with NVAF.

Objective To investigate the effect of Ticagrelor on four items of thrombi and platelet function in elderly patients with acute coronary syndrome (ACS) after complex percutaneous coronary intervention (PCI). Methods Sixty elderly patients with ACS treated by complex PCI in Guidong People’s Hospital, Guangxi Zhuang Autonomous Region, from January 2020 to December 2021 were selected for this study, and the patients were divided into control group (30 cases) and study group (30 cases) by random number table method. The patients were treated with conventional medication after complicated PCI, including the control group treating by Clopidogrel combined with Aspirin and the study group treating by Ticagrelor combined with Aspirin, and the duration of treatment in both groups was six months. The four items of thrombi and platelet function were compared between the two groups before medication and one month after medication, and the occurrence of adverse cardiovascular events in the two groups during six months after complex PCI was recorded. Results One month after medication, the levels of thrombin-antithrombin, thrombomodulin, tissue-type plasminogen activator- plasminogen activator inhibitor 1 complex, and plasmin- α2-plasmin inhibitor complex in the two groups were lower than those before medication, and those in the study group were lower than those in the control group (P<0.05). One month after medication, the arachidonic acid inhibition rate, adenosine diphosphate inhibition rate, adenosine diphosphate induced fibrin clot intensity, and platelet reactivity index in the two groups were higher than those before medication, and those in the study group were higher than those in the control group (P<0.05). Within six months of treatment, the total incidence of cardiovascular adverse events in the study group was lower than that in the control group (P<0.05). Conclusion Ticagrelor combined with Aspirin has good anti-platelet effect in elderly ACS patients after complex PCI, which can improve platelet function and reduce the incidence of adverse cardiovascular events.

Objective To investigate the efficacy and safety of atorvastatin plus low-molecular-weight dextran volume expansion in the treatment of acute cerebral infarction. Methods A total of 160 cases of acute cerebral infarction admitted to Wujin Hospital of Traditional Chinese Medicine in Changzhou from February 2018 to February 2022 were selected and they were divided into study group and control group by random number table method, with 80 cases in each group. The control group received routine treatment and Atorvastatin Calcium treatment, while the study group received low-molecular-weight dextran volume expansion treatment. After one week of treatment, patients of both groups were evaluated for efficacy, serum interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) were compared between the two groups, and activities of daily life (ADL), Barthel index (BI), and Montreal cognitive assessment (MOCA) scores of cognitive function were compared between the two groups. Results The clinical efficacy of the study group was better than that of the control group, and the difference was statistically significant (P<0.05). After one week of treatment, the levels of TNF-α and IL-6 in two groups were lower than those before treatment, and the study group was lower than control group, the differences were statistically significant (P<0.05). After one week of treatment, MOCA, ADL, and BI scores in two groups were higher than those before treatment, and the study group was higher than control group, the differences were statistically significant (P<0.05). There was no significant difference in the total incidence of adverse reactions between two groups (P>0.05). Conclusion The intensive treatment of low-molecular-weight dextran volume expansion for acute cerebral infarction is helpful to promote the rapid recovery of patients, reduce the level of inflammation, improve cognitive function, improve the quality of life, and reduce the incidence of adverse reactions.

Objective To investigate the rehabilitation value of lower extremity rehabilitation robot for stroke patients with severe hemiplegia. Methods A total of 130 stroke patients with severe hemiplegia admitted to Department of Rehabilitation Medicine, the First Affiliated Hospital of Guangxi University of Science and Technology from January 2019 and December 2020 were selected as study subjects. They were divided into group A (65 cases) and group B (65 cases) by random number table method. group A was treated with folic acid + conventional rehabilitation training, and group B was treated with folic acid + conventional rehabilitation training + lower extremity rehabilitation robot, the course of treatment was six months. The motor function assessment scale (MAS) score, Barthel index, National Institutes of Health stroke scale (NIHSS) score, and anxiety and depression were compared between two groups before intervention, three and six months after intervention; patients in both groups were followed up by telephone for 180 d, and short form 36 (SF-36) scores were compared between two groups. Results Overall analysis showed that MAS score, Barthel index, NIHSS score between groups, time, and interaction were statistically significant between the two groups (P<0.05). Intra-group comparison: MAS score, Barhtel index, and NIHSS score were pairwise compared at different time points in two groups, and the differences were statistically significant (P<0.05). Inter-group comparison: after six months of intervention, MAS score and Barthel index in group B were higher than those in group A, and NIHSS score was lower than those in group A, and the differences were statistically significant (P<0.05). The overall analysis showed that there were statistically significant differences between groups, time, and interaction of self-rating anxiety scale (SAS) and self-rating depression scale (SDS) scores (P<0.05). Intra-group comparison: SAS and SDS scores were pairwise compared at different time points in the two groups, and the differences were statistically significant (P<0.05). Inter-group comparison: after three and six months of intervention, SAS and SDS scores in group B were lower than those in group A, and the differences were statistically significant (P<0.05). After intervention, the scores of physiological function, physiological function, physical pain, vitality, social function, emotional function, mental health, and total score of two groups were higher than those before intervention, and group B was higher than group A, and the differences were statistically significant (P<0.05). Conclusion Lower extremity rehabilitation robot can significantly improve the motor function of the affected limbs, nerve damage, and adverse psychology of stroke patients with severe hemiplegia, and improve the quality of life of patients, which is worth promoting.

Objective To explore the intervention effect of exercise training on stroke patients with hemiplegia under hybrid reality technology. Methods A total of 265 stroke patients with hemiplegia admitted to the Department of Rehabilitation Medicine, the First Affiliated Hospital of Xi’an Medical University from January 2019 to December 2022 were selected, they were divided into study group (132 cases) and control group (133 cases) by random number table method. The control group received routine treatment and limb functional exercise training, while the study group received mixed reality technology exercise training on the basis of the control group. Both groups were intervened for three months. The lower limb motor function, balance function, serum biochemical indicators, and brain nerve function were compared between the two groups before and after intervention. Results During the intervention process, two cases were excluded in the study group (one case with poor cooperation, one case with unstable condition), and four cases were excluded in the control group (three cases withdrew for their own reasons, one case with poor cooperation). Finally, 130 cases were included in the study group and 129 cases in the control group, respectively. After intervention, the scores of lower limb Fugl-Meyer assessment scale (FMA), Berg balance scale (BBS), serum nerve growth factor (NGF), and brain-derived neurotrophic factor (BDNF) in the two groups were higher than those before intervention, and the values of δ/α power ratio (DAR) and brain symmetry index (BSI) were lower than those before intervention, and the FMA score, BBS score, and serum NGF and BDNF levels in the study group were higher than those in the control group, and the values of DAR and BSI were lower than those in the control group(P<0.05). Conclusion Mixed reality technology exercise training can improve lower limb motor function and balance ability in stroke patients with hemiplegia, and promote brain function remodeling.

Objective To observe the therapeutic effect of transcranial direct current stimulation (tDCS) combined with closed kinetic chain (CKC) training on upper limb motor dysfunction after stroke. Methods A total of 60 upper limb motor dysfunction after stroke patients admitted to Xuzhou Central Hospital and Xuzhou Rehabilitation Hospital in Jiangsu Province from January 2021 to January 2022 were selected and they were divided into the tDCS group, CKC group, and combined group by the random number table method, with 20 cases in each group. All three groups received conventional rehabilitation treatment, on this basis, tDCS group was added tDCS treatment, CKC group was added CKC treatment, and the combined group was added tDCS combined with CKC treatment. Fugl-Meyer upper limb scale (FMA-UE), Hong Kong version of hemiplegic upper limb function test scale (FTHUE-HK) and modified Barthel index scale (MBI) were evaluated by the same therapist before and after treatment. Cortical latency (CL) and central motor conduction time (CMCT) of motor evoked potentials were measured respectively. Results During the study period, one patient in the CKC group did not complete all treatment due to personal reasons and was excluded. After treatment, the scores of FMA-UE, FTHUE-HK, and MBI in three groups were higher than those before treatment, and the combined group was higher than tDCS group and CKC group, and the differences were statistically significant (P<0.05). After treatment, CL and CMCT in the three groups were shorter than those before treatment, and the combination group was shorter than tDCS group and CKC group, and the differences were statistically significant (P<0.05). Conclusion tDCS combined with CKC training has a good effect on improving upper limb motor function after stroke, and the effect is superior to tDCS and CKC training alone.

Objective To explore the clinical effect of acupoint massage on hemiplegia patients with cerebral infarction. Methods A total of 102 hemiplegia patients with cerebral infarction treated in Liyang People’s Hospital of Jiangsu Province from June 2019 to June 2022 were divided into observation group and control group by random number table method, with 51 cases in each group. The control group was treated with Butylphthalide, and the observation group was treated with traditional Chinese medicine acupoint massage on the basis of the control group. After two months of treatment, the scores of the simplified Fugl-Meyer motor function scale (FMA), the maximum grip strength of the affected hand, the score of the National Institutes of Health neurological deficiency scale (NIHSS) and the level of homocysteine (Hcy) were compared between the two groups. Results There were significant differences in FMA scores of the two groups at different time points (P<0.05). After one and 2 months of treatment, FMA scores in observation group were higher than those in control group (P<0.05). After two months of treatment, the maximum grip strength of the affected side was increased in both groups, and the observation group was higher than the control group (P<0.05). There was significant difference in NIHSS score between the two groups at different time points (P<0.05). After two months of treatment, serum Hcy level in both groups was lower than before treatment, and the observation group was lower than the control group (P<0.05). Conclusion The application of acupoint massage in the auxiliary treatment of patients with cerebral infarction and hemiplegia is quite beneficial for promoting the recovery of motor function, and has a promising application prospect.

95 Effect of Raloxifene on negative symptoms, cognitive function, and quality of life in menopausal patients with chronic schizophrenia

Objective To study the effect of Raloxifene on negative symptoms, cognitive function, and quality of life in menopausal patients with chronic schizophrenia. Methods A total of 80 postmenopausal female schizophrenics with negative symptoms and long-term treatment with Clozapine from the outpatient department of Wuhu Fourth People’s Hospital, Anhui Province from January 2021 to January 2022 were selected as the research object. According to random number table method, they were divided into control group (40 cases) and study group (40 cases). The control group was treated with Clozapine, and the research group was treated with Clozapine and Raloxifene, the treatment lasted for 12 weeks. The positive and negative symptom scale (PANSS) score, scale for assessment of negative symptoms (SANS) score, serum estradiol (E2) levels, the repeatable battery for the assessment of neuropsychological status (RBANS) score, and generic quality of life inventory-74 (GQOLI-74) score were compared between two groups before and after treatment; the occurrence of adverse reactions during treatment was observed. Results After treatment, the SANS score and PANSS score of two groups were lower than those before treatment, and those in study group were higher than those in control group (P<0.05), and the E2 level of two groups was higher than that before treatment, and that in study group was higher than that in control group (P<0.05). After treatment, the scores of immediate memory, attention, verbal function, delayed memory, and visual span in two groups were higher than those before treatment, and those in study group were higher than those in control group (P<0.05). After treatment, the scores of material life status, social function, physical function, and psychological function of two groups were higher than those before treatment, and those in study group were higher than those in control group (P<0.05). There was no significant difference in the total incidence of adverse reactions between two groups (P>0.05). Conclusion Raloxifene can effectively relieve the negative symptoms of menopausal patients with chronic schizophrenia, improve the quality of life and cognitive function of patients, and has high safety.

99 Effect of Vitamin AD Drops combined with glucocorticoid inhalation in the treatment of mycoplasma pneumonia in children and its effect on immune function

Objective To observe the effect of Vitamin AD Drops combined with glucocorticoid inhalation in the treatment of mycoplasma pneumonia in children and its effect on immune function. Methods A total of 92 children with mycoplasma pneumonia in children admitted to Wuxi Maternal and Child Health Hospital, Jiangsu Province from January 2018 to October 2019 were selected as study subjects. They were divided into two groups by odd-even number labeling method, with 46 cases in each group. Both groups were treated with Azithromycin, the glucocorticoid group were treated with inhalation glucocorticoid, and combined group were treated with Vitamin AD Drops based on glucocorticoid group, the course of treatment was two weeks. The clinical effect of two groups was compared; symptoms and signs improvement time, immune function, regulatory T cells/helper T cells 17 (Treg/Th17) were compared between two groups; patients were followed up for one year and the recurrence was recorded in both groups. Results In glucocorticoid group, three cases were shed, and 43 cases were enrolled. The disappearance time of fever, asthma, cough, and wet rales in combination group was shorter than that in glucocorticoid group (P<0.05); there was no significant difference in the time of lung shadow disappearance between two groups (P>0.05). After treatment, CD3+, CD4+, and CD4+/CD8+ in both groups were higher than those before treatment, and CD8+ was lower than that before treatment, and CD3+, CD4+, and CD4+/CD8+ in combination group were higher than those in glucocorticoid group, and CD8+ was lower than that in glucocorticoid group (P<0.05). After treatment, Treg and Treg/Th17 were higher than those before treatment, and Th17 was lower than that before treatment, and Treg and Treg/Th17 in combination group were higher than those in glucocorticoid group, and Th17 was lower than that in glucocorticoid group (P<0.05). There was no significant difference in clinical efficacy between two groups (P>0.05). The recurrence rate of combined group was lower than that of glucocorticoid group, and the treatment time when recurrence was shorter than that of glucocorticoid group (P<0.05). Conclusion Vitamin AD Drops combined with glucocorticoids can promote the disappearance of symptoms and signs and reduce recurrence of mycoplasma pneumonia in children, which may be related to the regulation of immune function.

Objective To investigate the clinical effect of Jian’er zhixie massage technology in the treatment of children persistent and chronic diarrhea. Methods A total of 60 children with persistent and chronic diarrhea treated in outpatient department of Wangjing Hospital of China Academy of Chinese Medical Sciences from June to December 2019 were selected as research objects. According to the order of treatment, they were divided into control group and observation group, with 30 cases in each group. The control group was treated with basic treatment and diet adjustment, and observation group was added Jian’er zhixie massage technology on the basis of control group, the treatment time was seven days. The clinical efficacy of two groups was compared; the score of frequency of stool, the characteristics of stool, and the total score of traditional Chinese medicine syndrome were compared between two groups before and after treatment. Results The clinical efficacy of observation group was better than that of control group, and the difference was statistically significant (P<0.05). After treatment, the score of frequency of stool, the characteristics of stool, and the total score of traditional Chinese medicine syndrome in observation group were lower than those before treatment; the score of characteristics of stool and the total score of traditional Chinese medicine syndrome in control group were lower than those before treatment, and the differences were statistically significant (P<0.05). After treatment, the score of frequency of stool, the characteristics of stool, and the total scores of traditional Chinese medicine syndrome in observation group were lower than those in control group, and the differences were statistically significant (P<0.05). Conclusion Jian’er zhixie massage technology is effective in the treatment of children persistent and chronic diarrhea, and can improve the traditional Chinese medicine syndrome as a whole. This method is simple and easy to operate, good compliance, is a better treatment for children persistent and chronic diarrhea.

107 Relationship between the expression of long non-coding RNA OSTN-AS1 and microRNA-1207-3p in prostate cancer tissues and clinicopathological parameters and prognosis

Objective To investigate the expression of long non-coding RNA (lncRNA) OSTN-AS1 and microrNA (miR) - 1207-3p in prostate cancer (PCa) tissues and their relationship with clinicopathological parameters and prognosis. Methods A total of 102 PCa patients admitted to Beijing Ditan Hospital Capital Medical University from January 2015 to December 2017 were selected. Real-time fluorescence quantitative PCR was used to detect the expression of lncRNA OSTN-AS1 and miR-1207-3p in cancer tissues and adjacent tissues, and the relationship between the expression and clinicopathological characteristics was analyzed. According to the mean expression of lncRNA OSTN-AS1 and miR-1207-3p in cancer tissues, patients were divided into low lncRNA OSTN-AS1 expression group (<3.26, 50 cases) and high lncRNA OSTN-AS1 expression group (≥3.26, 52 cases); and low miR-1207-3p expression group (<3.69, 49 cases) and high miR-1207-3p expression group (≥3.69, 53 cases). Follow-up for five years was conducted to analyze the relationship between lncRNA OSTN-AS1, miR-1207-3p, and prognosis. Cox regression analysis of the factors affecting the prognosis of PCa patients. Results The expressions of lncRNA OSTN-AS1 and miR- 1207-3p in PCa tissues were higher than those in paracancer tissues, and the differences were statistically significant (P<0.05). The expression levels of lncRNA OSTN-AS1 and miR-1207-3p in PCa tissues with different TNM stages, Gleason score and lymph node metastasis were statistically significant (P<0.05). The 5-year cumulative survival rate of lncRNA OSTN-AS1 high expression group was lower than that of lncRNA OSTN-AS1 low expression group (P<0.05). The 5-year cumulative survival rate of miR-1207-3p high expression group was lower than that of miR-1207-3p low expression group (P<0.05). Cox regression analysis showed that TNM stage Ⅲ, Gleason score > seven points, lymph node metastasis, lncRNA OSTN-AS1 high expression, and miR-1207-3p high expression were independent risk factors for the prognosis of PCa patients (P<0.05). Conclusion The expression of lncRNA OSTN-AS1 and miR-1207-3p increased in PCa tissues, which were related to Gleason score, TNM stage, lymph node metastasis, and prognosis survival, and involved in tumor progression in PCa.

112 Effect of Angelica Buxue Decoction on patients with early lung cancer after resection

Objective To investigate the effect of Danggui Buxue Decoction on patients with early lung cancer after resection. Methods By random number table method, a total of 94 patients with early lung cancer admitted to Huzhou Hospital of Traditional Chinese Medicine in Zhejiang Province from February 2021 to September 2022 were divided into experimental group and control group, with 47 cases in each group. All patients underwent partial pulmonary resection. The control group was given Pemetrexed combined with Cisplatin, and the experimental group was given angelica blood-supplementing decoction. The clinical efficacy, traditional Chinese medicine syndrome score, tumor marker level, and treatment safety of the two groups were compared. Results The clinical effect of the experimental group was better than that of the control group, and the difference was statistically significant (P<0.05). After treatment, the scores of coughing hemoptysis, chest depression, fatigue, and tongue purpleness in the two groups were lower than before treatment, while those of the experimental group were lower than those of the control group, the differences were statistically significant (P<0.05). After treatment, the levels of phosphatidylkinol 3-kinase, protein kinase, soluble tumor necrosis factor receptor in the two groups were lower than before treatment, while those of the experimental group were lower than those of the control group, and the differences were statistically significant (P<0.05). There was no significant difference in the total incidence of adverse reactions (P>0.05). Conclusion Angelica Buxue Decoction has good therapeutic effect on patients with early lung cancer after resection, can improve the symptoms of patients and reduce the level of tumor markers.

Objective To investigate the correlation of serum soluble tumor necrosis factor-associated apoptosis-inducing ligand (sTRAIL), galectin-3, nesfatin-1 with chronic obstructive pulmonary disease (COPD) complicated with depression. Methods A total of 173 hospitalized patients with acute exacerbation of COPD admitted to the First Affiliated Hospital of Hebei North University from July 2019 to May 2021 were selected, including 103 patients in the simple COPD group and 70 patients in the COPD combined with depression group. Clinical data and serum levels of sTRAIL, galectin-3, and nesfatin-1 of the two groups were recorded and compared. Logistic regression model was used to analyze the influencing factors of COPD combined with depression, and receiver operating characteristic (ROC) curve was further drawn to evaluate the prediction efficiency. Results The COPD assessment test score, the proportion of poor treatment compliance, the proportion of chronic diseases, the proportion of patients living alone, and the serum levels of sTRAIL, galectin-3, and nesfatin-1 in the COPD combined with depression group were higher than those in the simple COPD group, the percentage of forced expiratory volume in first second to forced vital capacity (FEV1/FVC), the percentage of forced expiratory volume in the first second to the expected value (FEV1pred), and the proportion of employees medical insurance in the COPD combined with depression group were lower than those in the simple COPD group, and the differences were statistically significant (P<0.05). The results of multivariate analysis showed that FEV1/FVC, FEV1pred, treatment compliance, medical insurance type, and serum sTRAIL, galectin-3, and nesfatin-1 were the influencing factors of COPD combined with depression (P<0.05). ROC curve analysis showed that serum sTRAIL, galectin-3, and nesfatin-1 alone and in combination had certain predictive value for COPD complicated with depression (P<0.05). Conclusion Depression in patients with COPD is associated with a variety of factors, serum sTRAIL, galectin-3, and nesfatin-1 joint detection merge depression in patients with COPD diagnosis has a certain value.

Objective To explore the effect and cost-effectiveness of autologous platelet-rich gel combined with negative pressure drainage on refractory diabetic foot in different treatment cycles. Methods A total of 85 patients with refractory diabetic foot admitted to the Second People’s Hospital of Lianyungang, Jiangsu Province from April 2019 to April 2022 were selected and divided into control group (43 cases, autologous platelet-rich gel combined with negative pressure drainage once a week) and observation group (42 cases, autologous platelet-rich gel combined with negative pressure drainage once every two weeks) by random number table method. Wound healing time, hospital stay, and clinical efficacy of the two groups were analyzed, serum inflammatory factors and antithrombine (AT) levels were analyzed before and after treatment, adverse reactions during treatment were recorded, and cost-effectiveness ratio was calculated. Results There were no significant differences in wound healing time, hospital stay, clinical efficacy, and total incidence of adverse reactions between the two groups (P>0.05). After four weeks of treatment, the levels of interleukin-6 and tumor necrosis factor-α in both groups were lower than before treatment, and the levels of AT were higher than those before treatment (P<0.05). After four weeks of treatment, there was no significant difference in the levels of interleukin-6, tumor necrosis factor-α, and AT between the two groups (P>0.05). The cost-effectiveness ratio of observation group was lower than that of control group. Conclusion Biweekly autologous platelet-rich gel combined with negative pressure drainage in patients with refractory diabetic foot doesn’t reduce clinical efficacy, but reduces cost-effectiveness ratio.

Objective To reevaluate the systematic review and meta-analysis of Duhuo Jisheng Decoction in the treatment of knee osteoarthritis (KOA). Methods Systematic reviews and meta-analyses on Duhuo Jisheng Decoction in the treatment of KOA were collected from CNKI, Wanfang Data, VIP, CBM, PubMed, Web of Science, the Cochrane Library, and Prospero. The search period was from inception until December 2022. Preferred reporting items for systematic reviews and meta-analyses (PRISMA) statement was used to evaluate report quality, AMSTAR-2 scale was used to evaluate methodological quality, and GRADE system was used to grade the evidence quality of outcome indicators. Results A total of eight literatures were included, the control group involved western medicine and acupuncture, and the experimental group used Duhuo Jisheng Decoction alone and Duhuo Jisheng Decoction on the basis of the control group. PRISMA statement scores of eight literatures were ≥20 points, and the evaluation results of AMSTAR-2 scale showed that many key items were substandard, and the methodological quality of the literatures was extremely low. The evaluation results of the GRADE system showed that there was no high quality of evidence and the overall quality of evidence was low. Conclusion The overall methodology and evidence quality of meta-analysis related to the clinical effect of Duhuo Jisheng Decoction in the treatment of KOA are not high, the study conclusions have certain limitations, and the evaluation of safety is less. Physicians should be cautious in the use of clinical decision- making, and future relevant studies need to further strengthen the standardization.

Professor Liu Guobin points out that the etiology and pathogenesis of this disease are blood stasis and obstruction based on rich clinical experience. Removing decay and promoting new growth as a classic theory for external treatment of ulcers in traditional Chinese medicine, Professor Liu Guobin has developed new connotations, removing necrosis generates new blood, new veins, new flesh, and new skin. He also proposes the theory of “eliminating necrosis and promoting regeneration” to select the appropriate time for the treatment of ulcers during the repair process. The decay and rebirth of sores are not only opposing each other, but also mutually reinforcing and mutually beneficial. Treatment at different stages has different focuses. On the basis of inheriting the academic thought of Yanjing Zhao’s dermatology school in the treatment of papular urticaria, Professor Zhang Guangzhong believes that the initial manifestation is the wind evil and stagnation of Xuanfu, the imbalance of the lungs and spleen is the key pathological link, and the weakness of the spleen and stomach is the fundamental pathogenesis. On the basis of the conventional methods of clearing heat and removing blood stasis, cooling blood and detoxifying, dispelling wind and relieving itching, the treatment emphasizes slightly sweating and relaxing Xuanfu to relieve external symptoms, xuanfei jinyun spleen soil to remove dampness, tonifying spleen and stomach, nourishing yin and nutrient to strengthen the defense, and the combination of acupuncture and medicine, internal and external treatment, complement each other. Sudden deafness is a common emergency in otolaryngology, characterized by sensorineural hearing loss in a short period of time, often accompanied by vertigo, tinnitus, and other symptoms, mostly in middle-aged and elderly people, the incidence has been increasing in recent years, the treatment effect of simple western medicine is not ideal and there are certain adverse reactions. On the basis of summarizing predecessors’ experience and his own clinical practice, Professor Bai Peng believes that the pathogenesis of sudden deafness is Shaoyang’s adverse mechanism, mostly due to phlegm turbidness, blood stasis, and other pathological products blocking the ear opening, and clearing qi does not rise. Professor Bai Peng takes “restoring the function of the zang-fu organs and regulating the ear opening qi movement” as the general principle of treatment. By regulating the zang-fu organs, regulating the meridian-regulating the ear opening, and combining the “part” point selection, that is, the neck point, the limbs point, and the ear circumference point, the Shaoyang cardinal can be adjusted regularly to make the whole qi movement rise and fall, and the yin and yang are at peace. Objective To evaluate the quality of report, methodological quality, and evidence level of systematic review on the prevention and treatment of chemotherapy-related cardiotoxicity by traditional Chinese medicine injection. Methods The PubMed, EMBase, Cochrane Library, CNKI, VIP, Wanfang Data, China Biomedical Literature Service System database were searched, the time limited to the database establishment until April 31, 2022. The published articles on systematic reviews/meta-analyses of traditional Chinese medicine injection for the prevention and treatment of chemotherapy-related cardiotoxicity were collected. The PRISMA statement was used to evaluate the reporting quality of the included studies, the AMSTAR2 scale and the GRADE rating system were used to evaluate the methodological quality and the level of evidence. Results A total of nine articles were included, and PRISMA scores ranged from 13.5 to 23 points. There were shortcomings in scheme and registration, other analysis and funding sources. AMSTAR2 evaluation indicated that the quality rating of one article was very low, three articles were low, and five articles were intermediate. GRADE Evidence quality was graded from medium quality to very low quality. A total of 49 outcome indicators were included, of which 11 were of medium quality, 10 were of low quality, and 28 were of very low quality. Conclusion The existing evidence suggests that the systematic evaluation methodology and evidence quality of traditional Chinese medicine injection for prevention and treatment of chemotherapy-related cardiotoxicity are low, but the efficacy is positive. Compared with the use of chemotherapy drugs alone or placebo, the incidence of electrocardiogram abnormalities and left ventricular ejection fraction of the main outcome indicators suggest that traditional Chinese medicine injection can effectively improve the prevention and treatment of chemotherapy-related cardiotoxicity.

Objective To investigate the effect of Yisui Fuyong Decoction on dementia under the theory of “treating encephalopathy with yin constitution based on warming yang method”. Methods A total of 78 patients with senile dementia admitted to The Fourth Clinical Medical College, Chengdu University of Traditional Chinese Medicine from July 2020 to July 2022 were selected and they were divided into treatment group and control group according to random number table method, with 39 cases in each group. The control group was given Donepezil Hydrochloride Tablets, and the treatment group was given Yisui Fuyong Decoction on the basis of the control group. Both groups were treated for six months. The mini mental status examination (MMSE), Barthel index, serum homocysteine (Hcy), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), traditional Chinese medicine symptom score were compared between the two groups before and after treatment and clinical effect was compared after treatment. Results After treatment, MMSE and Barthel index scores in both groups were higher than before treatment, and those in the treatment group were higher than those in the control group, the differences were statistically significant (P<0.05). After treatment, the levels of Hcy, TNF-α, and IL-6 in two groups were lower than before treatment, and the levels of TNF-α and IL-6 in treatment group were lower than those in control group, the differences were statistically significant (P<0.05). There was no significant difference in Hcy level between the two groups (P>0.05). After treatment, the traditional Chinese medicine symptom score of the two groups was lower than that before treatment, and that of the treatment group was lower than that of the control group, the differences were statistically significant (P<0.05). The therapeutic effect of the treatment group was better than that of the control group, and the difference was statistically significant (P<0.05). Conclusion Under the theory of “treating encephalopathy with yin constitution based on warming yang method”, Yisui Fuyong Decoction can improve the effect of dementia and improve the clinical symptoms of patients, which is worthy of clinical application.

Objective To study the effect of adjuvant therapy with Fuling Sini Decoction on organ function and inflammation response in patients with severe sepsis after continuous blood purification treatment. Methods A total of 88 patients with severe sepsis in the Department of Critical Care Medicine, Taicang Traditional Chinese Medicine Hospital, Jiangsu Province from December 2017 to December 2021 were selected and they were divided into control group and observation group according to random number table method, with 44 patients in each group. The control group was treated with continuous blood purification, and the observation group was treated with Fuling Sini Decoction on the basis of the control group. The efficacy of the two groups was compared, and the traditional Chinese medicine syndrome score, organ function (glutamic-pyruvic transaminase [GPT], glutamic-oxaloacetic transaminase [GOT], total bilirubin [TB], serum creatinine [SCr], blood urea nitrogen [BUN], uric acid [UA], left ventricular ejection fraction [LVEF], left ventricular end diastolic diameter [LVEDD], left ventricular end systolic diameter [LVESD]), inflammatory response (interleukin-6 [IL-6], tumor necrosis factor-α [TNF-α], white blood cell count [WBC], C-reactive protein [CRP]) before and after three days of treatment were compared and mechanical ventilation time and hospital stay were counted. Results After three days of treatment, traditional Chinese medicine syndrome scores of both groups were decreased, and those of observation group was lower than those of control group (P<0.05). The curative effect of observation group was better than that of control group (P<0.05). After three days of treatment, GPT, GOT, TB, LVEDD, LVESD, SCr, BUN, and UA in both groups were lower than before treatment, and LVEF was higher than before treatment, and GPT, GOT, TB, LVEDD, LVESD, SCr, BUN, and UA in observation group were lower than control group, and LVEF was higher than control group (P<0.05). After three days of treatment, serum levels of IL-6, TNF-α, WBC, and CRP in both groups were decreased, and those in observation group was lower than those in control group (P<0.05). Mechanical ventilation time and hospital stay in observation group were shorter than those in control group (P<0.05). Conclusion Fuling Sini Decoction with continuous blood purification is effective in the treatment of severe sepsis. It can reduce traditional Chinese medicine syndrome score, improve organ function, reduce inflammation response, shorten mechanical ventilation time and hospital stay.

Objective To explore the effects of an online Balint group on burnout and emotion of psychiatric nurses. Methods A total of 60 nurses from the Mental Health Center Affiliated to Shanghai Jiao Tong University School of Medicine from April to July 2022 were selected as the study objects. They were divided into control group and intervention group by random number table, with 30 nurses in each group. The control group received routine care, and the intervention group participated in online Balint group activities once a week on the basis of the control group, both two groups were treated for 12 weeks. The nurses in the two groups were evaluated by the Maslach burnout scale-service version, communication ability rating scale, self-rating anxiety scale, self-rating depression scale, and self-emotional control scale before and after the study. Results After intervention, the scores of emotional fatigue and job apathy in intervention group were lower than those before intervention, and the scores of job accomplishment in the intervention group were higher than that before intervention (P<0.05); the scores of emotional fatigue and work indifference in the intervention group were lower than those in the control group, and the scores of work achievement were higher than those in the control group, the differences were statistically significant (P<0.05). After intervention, self-rating anxiety scale and self-rating depression scale in the intervention group were lower than those before intervention, while self-emotion control ability and communication ability were higher than that before intervention, the differences were statistically significant (P<0.05). The self-rating anxiety scale and self-rating depression scale in the intervention group were lower than those in the control group, and the self-emotion control ability and communication ability were higher than those in the control group, the differences were statistically significant (P<0.05). Conclusion The use of online Balint group intervention can improve job burnout and emotion of psychiatric nurses.

Objective To explore the application of failure mode and effect analysis model in prevention of postoperative complications after transfemoral intervention. Methods A total of 100 patients admitted to the Sixth People’s Hospital of Nantong, Jiangsu Province undergoing transfemoral intervention from December 2019 to January 2022 were selected by cluster sampling. They were divided into reference group and observation group by random number table method, with 50 cases in each group. The reference group was treated with routine nursing, and the observation group was treated with failure mode and effect analysis mode, the nursing quality and complications were compared between the two groups. Results After nursing, the nursing quality score of the observation group was higher than that of the reference group, and the difference was statistically significant (P<0.05). The incidence of complications in the observation group was lower than that in the reference group, and the difference was statistically significant (P<0.05). Conclusion Failure mode and effect analysis mode can promote the improvement of nursing quality after femoral artery intervention and effectively avoid the risk of postoperative complications.

Objective To investigate the effect of ADOPT model care combined with 5A care on self-care ability and survival quality of patients with severe cerebral hemorrhage. Methods A total of 92 patients with severe cerebral hemorrhage admitted to the Second Affiliated Hospital of Air Force Military Medical University from January 2021 to December 2022 were selected, they were divided into control group and observation group according to random number table method, with 46 cases in each group. Neurologic, functional function, complications, self-care ability, and quality of life were compared between the two groups. Results After nursing, the neurological impairment score of the two groups was lower than those before nursing, and the study group was lower than that of the control group (P<0.05). The motor function scores of upper and lower limbs in the two groups were higher than those before nursing, and the study group was higher than that of the control group (P<0.05). The total complication rate of study group was lower than that of control group (P<0.05). After nursing, the scores of self-care ability and total score and quality of life of the two groups were higher than those of before nursing, and the study group was higher than that of the control group (P<0.05). Conclusion ADOPT model care combined with 5A care can improve neurological and motor functions, reduce complications, and improve self-care ability and survival quality in patients with severe cerebral hemorrhage.

The main symptoms of primary hyperparathyroidism (PHPT) are bone and urinary system. The lower gastrointestinal bleeding as the primary symptom is rare. Few studies have reported similar cases at home and abroad, which are easily misdiagnosed as digestive tract diseases, lead to affect the early diagnosis and treatment, and even endanger the life of patients. This paper reports a case of PHPT with lower gastrointestinal bleeding as the primary symptom. The main clinical manifestations of this patient are abdominal pain and hematochezia. At present, pathological examination is the main means of diagnosis of PHPT, laboratory examination and imaging examination can provide a high diagnostic basis before diagnosis, and surgery is the main treatment method. No abdominal pain and hematochezia were observed in the patients after surgery. Through this case, it can be seen that lower gastrointestinal bleeding PHPT is the primary symptom, which is easily misdiagnosed and missed, and clinical understanding of this disease should be further strengthened.

Invasive pulmonary aspergillosis is difficult to distinguish from other pulmonary infections in clinic, and is easy to be misdiagnosed or missed. This paper report, a case of invasive pulmonary aspergillosis diagnosed by macrogene second generation sequencing, to investigate the diagnostic value of metagenomic next-generation sequencing in patients with invasive pulmonary aspergillosis. The main clinical manifestations of the patient were high fever, cough, and sputum with blood, after admission, the initial diagnosis was “bacterial pneumonia”, Cefoperazone sodium sulbactam sodium, Meropenem were given successively, the effect is not good, the relevant examination did not obtain sufficient etiological evidence, the metagenomic next-generation sequencing technology and pathological examination were performed, and the results indicated pulmonary infection of aspergillus, the antibiotic was changed to Voriconazole, and the patient was discharged after treatment. The possibility of fungal infection should be considered in patients with pulmonary infection who are not well treated by conventional antibiotics, if the pathogenic bacteria cannot be identified by routine examination, the diagnostic value of metagenomic next-generation sequencing technology should be emphasized.

Malignant lymphoma is a malignant proliferative tumor originating from immune cells of lymphoid tissue, western medicine treatment is mainly based on chemotherapy, which is easy to lose treatment and mistreatment in clinical practice, and it is a difficult disease, and there are many accompanying symptoms, easy to repeated attacks and other problems. The patient was diagnosed with malignant lymphoma more than one year ago. Professor Wang Junge believed that the patient suffered from gastric motility deficiency after chemotherapy, the deficiency of stomach fluid can not be cured, resulting in solid blood, therefore, the main line of treatment is “removing water knot”, after five diagnosis and treatment, the effect is obvious. This case reviews Professor Wang Junge’s experience in differentiating and treating malignant lymphoma with Zhongjing academic system. In order to provide reference for the clinical treatment of this kind of disease, improve the clinical effect.

186 A case of cloacogenic carcinoma at one point with literature review

Cloacogenic carcinoma is a rare malignant tumor of the anal canal, originating from the embryonic persistent cloaca remnant. This tumor has significant histological heterogeneity and can be manifested as a variety of histological types such as transitional cell carcinoma, basal-like squamous cell carcinoma, and adenoid cystic carcinoma, which is easy to be misdiagnosed clinically. In this paper, a case of cloacogenic carcinoma patient admitted to the First Hospital of Hebei Medical University in March 2022 was retrospectively analyzed, the main symptoms were anal distension, blood in stool, and increased frequency of stool and other symptoms, a neoplasms of mass type was seen near the anal margin of the rectum under endoscopy, and tumor angiogenesis was observed. The preoperative biopsy pathology showed low differentiation adenocarcinoma, and the postoperative pathological results showed cloacogenic carcinoma. The human papilloma virus test was positive, and the cervical examination showed no abnormalities. Postoperative radiotherapy (DT6000cGy/30f) was performed. After six months of follow-up, the patient had no signs of tumor recurrence. This case was misdiagnosed as poorly differentiated adenocarcinoma before surgery, which is related to the lack of recognition of the disease by clinicians. In order to improve the understanding of the disease, this article reviewed the literature from histopathological features, clinical manifestations, pathogenesis, and current progress of diagnosis and treatment.

190 Construction of clinical trials management modules based on hospital information system

In order to explore the clinical trial information management mode based on hospital information system (HIS), make up for the problems such as the inability to review clinical trial medical orders in real time, the need to advance related expenses in advance, and the omission of invoices in manual reimbursement, and improve work efficiency, Beijing Boai Hospital built a clinical trial management module based on the existing HIS system to optimize clinical trial follow-up and internal transfer processes, explore a unique way of clinical trial information management. The clinical trial module was implemented in 25 clinical trial projects from September 2021 to November 2022, the operation is simple, convenient to use and manage, and realizes real-time supervision of the clinical trial process. It solves various problems such as inaccurate, easy to lose and difficult to trace the paper application form information, avoids the confusion between clinical trial inspection costs and medical insurance costs, and effectively improves the level of clinical research information management.

194 Empirical study on the evaluation of health resource allocation and utilization efficiency of traditional Chinese medicine in Shaanxi Province based on DEA model from the perspective of revitalization and development of traditional Chinese medicine

Objective To study the current situation and efficiency of the allocation and utilization of traditional Chinese medicine health resources in Shaanxi Province under the background of the revitalization and development of traditional Chinese medicine, and to put forward reasonable and feasible countermeasures and suggestions. Methods Data envelopment analysis (DEA) was used to analyze and evaluate the allocation and utilization efficiency of traditional Chinese medicine health resources in Shaanxi Province. Results The average comprehensive efficiency, average pure technical efficiency and average scale efficiency of eight DMU from 2013 to 2020 were 0.997, 0.988 and 0.988, respectively. Among the eight DMU, three were DEA effective, two were weak DEA effective, and three were non-DEA effective. Conclusion The health resources and utilization efficiency of traditional Chinese medicine in Shaanxi Province are generally good, but there is still a large room for improvement. It is necessary to adjust the input-output structure to make the actual scale consistent with the optimal scale. It is suggested that health administration departments and hospitals should allocate health resources according to demand, pay attention to personnel training and efficiency evaluation, and improve the efficiency of resource allocation and utilization.
