DOI: 10.20047/j.issn1673-7210.2023.16.01
Objective To study the effect of amphotericin B on Candida and Lactobacillus biofilm. Methods From July 2022 to August 2022, four strains of Candida and four strains of Lactobacillus from the vagina of patients in Beijing Obstetrics and Gynecology Hospital, Capital Medical University were selected to determine their biofilm formation ability at different stages and observe their biofilm formation. The minimum inhibitory concentration (MIC) and minimum biofilm inhibitory concentration (MBIC) of amphotericin B on Candida and lactobacillus were determined respectively, and the effect of amphotericin B on the biofilms was observed under scanning electron microscope. Results The biofilm formation ability of Candida and Lactobacillus at 48 h was higher than that at 24 h (P<0.05), and the biofilm formation ability of Lactobacillus No.7 at 48 h was higher than that at 72 h (P<0.05). The MIC of amphotericin B against Candida albicans was 0.125 00 μg/ml. When the concentration of amphotericin B was 2 μg/ml, the biofilm forming ability of Candida No.1, No.3 and No.4 was lower than that of the positive control group (amphotericin B 0 μg/ml)(P<0.05), and there was no difference between them and the subsequent concentrations (P>0.05), the MBIC of amphotericin B against Candida albicans was 2 μg/ml. Amphotericin B had no inhibitory effect on Lactobacillus crispatus. The biofilm forming ability of Lactobacillus No.7 at concentration of 1, 8, and 16 μg/ml was higher than that of positive control group (P<0.05), and the other Lactobacillus crispatus had no difference in biofilm forming ability at different concentrations (P>0.05).  Conclusion Candida albicans and Lactobacillus crispatus had the strongest biofilm forming ability at 48 h. Amphotericin B has strong antibacterial ability to Candida albicans, but it needs a larger dose to destroy the biofilms of Candida albicans which  have formed. It has no inhibitory effect on Lactobacillus crispatus and biofilms destructive effect.

DOI: 10.20047/j.issn1673-7210.2023.16.04
Objective To investigate the effects of vitamin D (VitD) intervention on the balance regulation of peripheral blood T lymphocytes and intestinal flora in young rats with bronchial asthma (hereinafter referred to as “asthma”). Methods Thirty SPF wild-type BALB/c female mice (4.09 - 6.13 g) aged seven days were selected as the study objects, and they were divided into asthma model group (15 rats) and VitD intervention group (15 rats) by random number table method, another ten rats were selected as healthy control group. On the first and seventh day of the experiment, asthma model group and VitD intervention group were injected intraperitoneally with 10% ovalbumin (OVA) sensitizing solution 200 μl, while healthy control group was injected with the same amount of normal saline. On the 18th day, VitD intervention group was given VitD drops (35 μg/kg, per week, one every two days) for two weeks, and healthy control group and asthma model group were given the same amount of phosphate buffer. On the 19th day, asthma model group and VitD intervention group were given atomized inhalation of 3%OVA solution 10 ml for 20 min, while healthy control group was given atomized inhalation of normal saline, once a day for six days. After the experiment, the hematoxylin-eosin staining results of lung tissue sections in three groups were observed. Serum Th1/Th2 and Th17/ regulatory T cell (Treg) ratios were measured and calculated by flow cytometry. The bacterial community in feces of the three groups was analyzed by 16S rDNA sequencing method. Results In the healthy control group, the structure of the bronchial wall and alveolar wall of the lung tissue were complete, there was no exudation in the alveolar cavity, the alveolar epithelium was orderly, and the alveolar wall thickness was normal. In asthmatic model group, the structure of alveolar wall was damaged, alveolar epithelial cells were destroyed, mucosal hyperemia and edema were observed, and perivascular inflammatory cells were infiltrated. The inflammatory reaction of lung tissue in VitD intervention group was less. The Shannon index curve of microbial species in the three groups was flat, and there was no statistical significance in Simpson index among the three groups (P>0.05). PCoA diagram analysis showed that the distance between the asthma model group and the healthy control group was large, and the distance between the VitD intervention group and the asthma model group was close (P<0.05). The three groups of intestinal flora involved 15 divisions and 23 genera. Th1/Th2 ratio and the abundance of Actinobacteriota, Helicobacter, and  Colidextribacter in asthma model group were lower than those in healthy control group, Th17/Treg ratio and the abundance of Proteobacteria, Ligilactobacillus, and  Bacteroides of asthma model group were higher than those of healthy control group (P<0.05). Th1/Th2 ratio and the abundance of Actinobacteriota, Proteobacteria, Helicobacter, and Colidextribacter in VitD intervention group were higher than those in asthma model group, The Th17/Treg ratio and the abundance of Ligilactobacillus, Bacteroides in VitD intervention group were lower than those of the asthma model group (P<0.05), which were consistent with the result of heatmap analysis based on genera level. Conclusion VitD intervention can regulate the balance of peripheral blood T lymphocytes and change the intestinal flora of young mice with asthma, which may be a new entry point for the pathogenesis of this disease.

DOI: 10.20047/j.issn1673-7210.2023.16.08
Rotavirus is a kind of enterovirus, the mechanism of which causes diarrhea is very complicated, and there is no specific drug to treat it. Recent studies have shown that 5-hydroxytryptamine plays a major role in rotavirus diarrhea. By modulating the expression of 5-hydroxytryptamine and the function of the enteric nervous system and intestinal epithelial barrier that it affects, the symptoms of rotavirus diarrhea can be effectively reduced. The relationship between 5-hydroxy tryptamine and rotavirus diarrhea has not been systematically organized. This review combines the latest relevant studies to provide a theoretical basis for the development of new anti-rotavirus drugs, with the aim of guiding clinical treatment more accurately.

58 Advances in molecular signaling pathways related to endometrial adhesions treated by stem cells related molecular signaling pathways
MA Guohui1 HUANG Yanhong2▲
DOI: 10.20047/j.issn1673-7210.2023.16.12
Intrauterine adhesion (IUA) is an important cause of infertility due to endometrial injury and abnormal adhesion caused by infection, abortion and repeated intrauterine operations. At present, for the treatment of IUA, mild to moderate patients can still achieve a certain effect through traditional treatment, but for patients with severe or extensive endometrial injury, endometrial regeneration and functional recovery has become a clinical thorny problem. With the extensive research of stem cells, stem cells with low immunogenicity, proliferation and differentiation advantages are considered to be one of the most potential methods in the treatment of intrauterine adhesions. Previous studies have shown that the paracrine effect of stem cells can promote the formation of new blood vessels and improve intrauterine fibrosis by secreting growth factors and inhibiting inflammation in the lesion area. In this paper, we review the existing molecular mechanisms of stem cell therapy for intrauterine adhesions in order to provide guidance for future research.

DOI: 10.20047/j.issn1673-7210.2023.16.19
Objective To systematically evaluate the effects of traditional Chinese patent medicines and simple preparations Hedan Tablets on blood-lipid levels and inflammatory factors in patients with dyslipidemia. Methods Clinical randomized controlled trials of Hedan Tablets in the treatment of dyslipidemia were retrieved from PubMed, CNKI, VIP, Wanfang Data, and China Biomedical Literature Service System from inception to March 2022. The meta-analysis of the results of the included literatures was performed by RevMan 5.3 analysis software and Stata 16.0 software. Results A total of 15 clinical randomized controlled trials were included, including 1 227 patients. The results of meta-analysis showed that the total clinical effective rate of the experimental group was higher than that of the control group (OR=0.28, 95%CI: 0.19 to 0.44, P<0.000 01). Compared with experimental group, serum levels of total cholesterol (MD=0.65, 95%CI: 0.16 to 1.14, P=0.01), triglyceride (MD=0.84, 95%CI: 0.45 to 1.23, P<0.000 01), low density lipoprotein cholesterol (MD=0.76, 95%CI: 0.08 to 1.44, P=0.03), C-reactive protein (SMD=2.60, 95%CI: 0.50 to 4.69, P=0.02) were higher in the control group, and serum level of high-density lipoprotein cholesterol (MD=-0.41, 95%CI: -0.70 to -0.12, P<0.000 01) was lower in the control group. There was no significant difference in the total incidence of adverse reactions between the two groups (OR=1.11, 95%CI: 0.71 to 1.72, P=0.65). Meta-regression analysis showed that publication time, sample size, statin type, and medication duration did not affect the effects of Herdan Tablets on blood-lipid levels and inflammatory factors in patients with dyslipidemia. The results of sensitivity analysis showed that the meta-analysis was stable. The funnel plot was basically symmetrical and had no obvious publication bias. Conclusion Hedan Tablets is better than western medicine alone in improving blood-lipid and inflammatory response indexes in patients with dyslipidemia, and does not increase adverse reactions, the effect is worthy of affirmation.

DOI: 10.20047/j.issn1673-7210.2023.16.20
Objective To compare the ω-3 polyunsaturated fatty acid (ω-3 PUFA) expression in serum of patients with pulmonary nodules, and to explore the independent risk factors for benign and malignant pulmonary nodules and establish a malignant probability prediction model. Methods A total of 294 patients with pulmonary nodule with definite pathology admitted to the Department of Cardiothoracic Surgery of the First People’s Hospital of Neijiang City, Sichuan Province from January 2018 to December 2021 were selected, and the patients were randomly divided into modeling group (201 cases) and verification group (93 cases). The difference of serum expression of ω-3 PUFA in benign and malignant pulmonary nodules was compared in the modeling group. The influencing factors of malignant pulmonary nodules were analyzed by single factor and multiple factor, and the diagnosis and prediction model was established and verified. Results There were statistically significant differences in ω-3 PUFA, focal diameter, consolidation/tumorratio(CTR), short burr, vascular signs and pleural pull between benign and malignant pulmonary nodules in the modeling group (P<0.05). Logistic regression showed that ω-3 PUFA, CTR, short burr, vascular sign, and pleural pull were independent influencing factors for benign and malignant pulmonary nodules (P<0.05). The prediction model was established: P=ex/(1+ex).X=-5.020+ (0.042×ω-3 PUFA positive) + (1.613×CTR) + (1.185× short burr) + (1.081× pleural drag) + (0.936× vascular sign). The area under curve (AUC) of receiver operating characteristic (ROC) curve of this model was 0.837, and the sensitivity and specificity were 74.5% and 84.6%, respectively. The AUC of the ROC curve of verification group was 0.780. Conclusion Lung nodules with decreased serum ω-3 PUFA, CTR≥0.5, burr sign, vascular sign, and pleural pull need to be highly suspected of malignancy. The established diagnosis and prediction model has a good prediction of benign and malignant pulmonary nodules.

DOI: 10.20047/j.issn1673-7210.2023.16.21
Objective To investigate the relationship between serum phosphorylated neurofilament heavy subunit(pNF-H)level in patients with septic shock and the occurrence of delirium. Methods A total of 117 patients with septic shock admitted to the Second Affiliated Hospital of Hainan Medical University from January 2020 to March 2022 were selected. According to delirium diagnostic criteria, patients were divided into the 73 cases in non-delirium group and 44 cases in delirium group. Univariate analysis, binary logistic regression model analysis, and receiver operating characteristic(ROC) curve were used to explore the relationship between serum pNF-H level in patients with septic shock and the occurrence of delirium. Results The sequential organ failure assessment(SOFA) score, the proportion of mechanical ventilation,28-day mortality, and serum levels of pNF-H and interleukin-6(IL-6) in the delirium group were higher than those in the non-delirium group, while serum albumin levels were lower than those in the non-delirium group(P<0.05). According to binary logistic regression model analysis, the elevated value of SOFA score, mechanical ventilation, and the elevated levels of serum pNF-H and IL-6 were independent risk factors for the occurrence of delirium in patients with septic shock(P<0.05). According to ROC curve analysis, the area under the curve of pNF-H in predicting the occurrence of delirium in patients with septic shock were higher than those of IL-6 and SOFA score(P<0.05). Conclusion The elevated serum pNF-H level is an independent risk factor for the occurrence of delirium, which can be used as a predictor of delirium in patients with septic shock.

DOI: 10.20047/j.issn1673-7210.2023.16.26
Modern medicine regards cardiovascular disease combined with mental and psychological problems as double heart disease. Professor Zhao Haibin, the founder of the “double heart theory” of traditional Chinese medicine, finds that the late stage of double heart disease often leads to the same result and involves the two viscera of the heart and kidney, especially in the elderly, who are often accompanied by depression, deficiency of both heart and kidney yang qi in the later stage of heart disease. Therefore, this paper concludes that the pathogenesis of the late stage of double-heart disease belongs to the “stagnation” stage of “stasis and heat deficiency and stagnation”, that is, the damage of the heart and kidney Yang, the loss of the mind, the yang qi of upper jiao is insufficient, yin qi lower jiao is sufficient and the stagnation of the heart and qi.The treatment should be based on the prescription of the meridian, using syndrome differentiation of six channels theory and eight principles of Shaoyin meridian, the syndrome differentiation of zang-fu viscera and the two viscera of the heart and kidney, the combination of disease and syndrome, considering a syndrome by several diseases, diagnosis and treatment based on syndrome differentiation. The method should warm the heart and kidney, nourishing the heart and calming the mind, promoting blood and activating qi. Sini Decoction combined with self-designed “Shuangxin Decoction” addition and subtraction, the clinical effect proved remarkable.

126 Cui Yinglin’s experience in treating cerebral infarction from “deficiency, stasis, phlegm, and turbidity”
ZHOU Panpan1 CUI Yinglin2
DOI: 10.20047/j.issn1673-7210.2023.16.27
Cerebral infarction is a common disease in neurology, with high incidence rate and high mortality, which seriously threatens human health. The etiology and pathogenesis of cerebral infarction are complex and varied, doctors of all dynasties have different opinions. Professor Cui, who has been engaged in the study of internal medicine for many years, believes that the core pathogenesis of cerebral infarction is “deficiency of yuan qi, static blood blocking collateral”, and deficiency, stasis, phlegm, and turbidity are the main pathological factors. Professor Cui proposes to take “benefiting qi for activating blood circulation, removing turbidity for removing obstruction in collaterals” as the basic treatment principles. Clinical treatment of cerebral infarction not only pays attention to the deficiency, stasis, phlegm, turbidity single disease evil nature changes, but also pay attention to their mutual combination of the change of pathogenesis. Therefore, when applying the basic principle of “benefiting qi for activating blood circulation, removing turbidity for removing obstruction in collateral”, the prescription Yiqi Tongmai Formula is formulated by add or subtract as required according to the course of disease, the changes of syndromes and years of clinical experience. The effect is remarkable when it is used in clinic.

130 Analysis on the idea of prevention and treatment of epidemic disease in Zhouhou Beiji Fang
JI Yongkuan CHEN Guosen NI Ruifan LIANG Yanlin ZHANG Qian’ao LIU Guo
DOI: 10.20047/j.issn1673-7210.2023.16.28
The epidemic treatment ideas in the epidemic chapter of Zhouhou Beiji Fang are reflected in three aspects: understanding of epidemic diseases, epidemic prevention methods, and drug characteristics. The book believes that the epidemic is caused by “pestilential qi” and “gui du”. The book attaches importance to the prevention and control of epidemic diseases, and uses methods such as household medicine, pre-made pill scattering, nasal administration, powder, and burning and smoking to diagnose and treat epidemics as soon as possible to avoid the spread of epidemics. The nine prescriptions for curing epidemic diseases in the article are used by later doctors. The author refers to the herbal monographs as Great Dictionary of Chinese Medicine and Mingyi Bielu to analyze the characteristics and efficacy of the drug, and summarize the effects of those prescriptions as warming yang, sha gui, detoxifying, and removing dampness. According to modern pharmacological studies, the herbs of epidemic prescriptions can exert pharmacological effects such as cardiotonic action, anti-infection, anticonvulsant, antiviral, antibacteria, anti-anxiety, and depression. By summarizing the epidemic control ideas of Zhouhou Beiji Fang, those theory has value for the study of the historical evolution of the epidemic treatment theory and drug characteristics of Chinese medicine epidemic treatment.

137 Professor Hu Guoheng’s experience in the treatment of endogenous cough based on the theory of “liver is born on the left and lung is hidden on the right”
CHEN Ya1 WANG Jinxi1 HU Guoheng1 YUAN Hua1 LIU Yingying1 XIE Jinru2 PIAO Meihong3 HE Piao2
DOI: 10.20047/j.issn1673-7210.2023.16.30
Endogenous cough is one of the most common diseases in internal medicine, its high incidence and recurrence rate is a clinical problem. Through many years of clinical diagnosis, Professor Hu Guoheng attributes the key pathogenesis to the reversed flow and disorder of liver and lung qi movement. Combined with the classic theory of “liver is born on the left and lung is hidden on the right”, he proposes the basic treatment of “harmonizing liver and spleen, opening stagnation, and dispersing excess”, which has remarkable effects. Endogenous cough is mostly caused by the dysfunction of zang-fu viscera, leading to the disorder of ascending and descending of yin and yang qi movement. The ascending and descending of liver and lung play a key role in the lifting axis, so it is mainly to “harmonizing liver and spleen, opening stagnation, and dispersing excess”, while not forgetting other zang-fu viscera. According to its deviation and impact on the running degree of qi, blood and body fluid, the overall grasp of ascending and descending of qi movement, which reflects the holistic traditional Chinese medicine thinking of syndrome differentiation.

141 Li Dongyuan’s discussion of the pathogenesis and treatment of polycystic ovary syndrome based on the theory of “rising of stomach qi”
KONG Lingyuan LI Xiuyang
DOI: 10.20047/j.issn1673-7210.2023.16.31
The theory of “rising of stomach qi” put forward by Li Dongyuan, a famous doctor of the Jin Dynasty, can be used to explain the pathological changes in polycystic ovary syndrome. Therefore, this paper tries to summarize the theory of “rising of stomach qi” and discuss the pathogenesis and treatment of polycystic ovary syndrome. Based on the analysis of the theory of “rising of stomach qi”, it can be seen that the basic pathogenesis of polycystic ovary syndrome is a malfunction of the rising of stomach qi, combined with liver qi stagnation, kidney essence qi deficiency, yin fire flaming upward and phlegm-dampness abundance inside. In the treatment of polycystic ovary syndrome, the primary treatment method is to rise the stomach qi, and according to the specific symptoms of the disease, methods such as regulating the rise of qi, tonifying the middle and benefiting the qi, removing heat with sweet-warm natured medicine, and benefiting the stomach to remove dampness are used. The theory of “rising of stomach qi” provides a new way of thinking for the clinical diagnosis and treatment of polycystic ovary syndrome and it is worth further exploration.

Objective To investigate the application effect of rectal magnetic resonance imaging (MRI) combined with endorectal ultrasound (ERUS) in diagnosis of complete response after neoadjuvant chemotherapy for rectal cancer. Methods A total of 66 patients with rectal cancer who received neoadjuvant chemotherapy and radical surgery in the First Affiliated Hospital of Hebei North University from January 2020 to December 2021 were selected. Rectal MRI and ERUS examinations were performed before and the seventh week after neoadjuvant chemotherapy to evaluate whether the patients had complete response. Pathological examination was used as the gold standard. The consistency of different tests in diagnosing complete response were compared with the gold standard. Results Pathological results showed that 22 of 66 patients with rectal cancer achieved complete response after neoadjuvant chemotherapy. There was no statistical significance between MRI and ERUS in the diagnosis of complete response after neoadjuvant chemotherapy and the gold standard (P>0.05), while MRI combined with ERUS was consistent with the gold standard (Kappa=0.400, P<0.05). The accuracy, specificity, and sensitivity of MRI combined with ERUS in the diagnosis of complete response after neoadjuvant chemotherapy in rectal cancer patients were 75.76%, 90.91%, and 45.45%. Conclusion After neoadjuvant chemotherapy for rectal cancer, the accuracy of simple MRI and ERUS in the diagnosis of complete response is not high, and the combined diagnosis has higher specificity and lower sensitivity than single diagnosis. For patients with non-complete response, radical surgery is simply performed. For patients with complete response, colonoscopy and digital examination are recommended, and subsequent treatment is implemented with caution.
