DOI: 10.20047/j.issn1673-7210.2022.36.01
Objective To investigate the biological roles of FAM83H and its natural antisense transcript FAM83H-AS1 in the occurrence and development of tumor, and the regulatory relationship between them. Methods The expression and prognosis of FAM83H and FAM83H-AS1 in various tumor tissues were analyzed by GEPIA online tool. Logarithmic growth stage A549 and SKOV3 cell lines were selected for transfection and divided into blank control group (control group), no-load control group (siNC group) and experimental group (siFAM83H group, SIFAM83H-AS1 group). The cell proliferation, apoptosis, and migration after FAM83H and Fam83H-AS1 knockdown were compared in each group. Results The expressions of FAM83H and FAM83H-AS1 mRNA were up-regulated in various tumor tissues, including lung adenocarcinoma and ovarian serous cystadenocarcinoma. In lung adenocarcinoma, the survival curves of FAM83H mRNA high expression group and FAM83H low expression group were compared, and the difference was statistically significant (P<0.05). In A549 and SKOV3 cells, the expressions of FAM83H mRNA and protein in siFAM83H group were lower than those in siNC group (P<0.05); the expression of FAM83H-AS1 mRNA in siFAM83H-AS1 group was lower than that in siNC group (P<0.05); cell proliferation in siFAM83H group was lower than that in siNC group after transfection for 24, 48 and 72 h (P<0.05); the cell proliferation ability of siFAM83H-AS1 group was lower than that of siNC group after transfection for 24, 48 and 72 h (P<0.05); the apoptosis number of siFAM83H group and siFAM83H-AS1 group was higher than that of siNC group (P<0.05); the number of cell migration in siFAM83H group and siFAM83H-AS1 group was lower than that in siNC group (P<0.05). After FAM83H knockdown, there was no significant difference in sIFAM83H-AS1 mRNA between siFAM83H group and siNC group (P>0.05); after FAM83H-AS1 knockdown, there was no statistical significance in FAM83H mRNA and protein between siFAM83H-AS1 group and siNC group (P>0.05). Conclusion FAM83H and FAM833H-AS1 play the role of oncogenes in the occurrence and development of lung adenocarcinoma and ovarian cancer, but no mutual regulatory relationship has been found between them, and the specific mechanism remains to be further studied.

DOI: 10.20047/j.issn1673-7210.2022.36.04
Objective To summarize the experience of traditional Chinese medicine in treating non-proliferative diabetic retinopathy (NPDR) based on data mining and network pharmacology. Methods Literatures of treating NPDR with traditional Chinese medicine from CNKI and Wanfang Data were searched and collated, and the limited time was from inception to December 2020. Traditional Chinese medicine inheritance computing platform V 3.0 was used to build a prescription database, and the frequency, flavor, meridian transformation, and efficacy of related prescriptions were counted. Association rules of related drug pairs were analyzed, and medication rules and core compatibility of traditional Chinese medicine treatment of NPDR were summarized. For the core drug pairs, the traditional Chinese medicine systems pharmacology database and analysis platform, traditional Chinese medicine integrative database, Uniprot, and GeneCards were used to construct the “ingredient-target” network, and the protein-protein interaction network was constructed with STRING database and Cytoscape software, and molecular docking was carried out. GO classification enrichment analysis and KEGG pathway enrichment analysis were performed by R language. Results According to the analysis of 107 prescriptions of NPDR treated by traditional Chinese medicine, ten drugs were commonly used, among which Radix Astragali seu Hedysari, Radix Rehmanniae Recens, and Radix Notoginseng were the most frequently used. The four nature of drugs were mainly cold, flat, and warm, the five flavours were mainly bitter and sweet, the channel tropism were mainly liver, lung, and kidney channel. There were seven drug pairs with high frequency of use, among which Radix Astragali seu Hedysari-Radix Rehmanniae Recens, Radix Astragali seu Hedysari-Radix Notoginseng, and Radix Astragali seu Hedysari- Radix Notoginseng ranked the top three. Network pharmacological analysis of the Radix Astragali seu Hedysari-Radix Rehmanniae Recens drug pair showed that the main components included quercetin, kaempferol, formononetin, isorhamnetin, etc., and the main targets included TP53, SERPINE1, VEGFA, TNF, etc. KEGG and GO enrichment pathways showed that the active components of traditional Chinese medicine were related to cell proliferation, cell response to chemical stress, and cell response to lipopolysaccharide. The free energy calculation of core target protein and core active ingredient showed that the core active ingredient in Radix Astragali seu Hedysari-Radix Rehmanniae Recens drug pair (quercetin, kaempferol) had a good binding effect with the key core target in NPDR. Conclusion Radix Astragali seu Hedysari-Radix Rehmanniae Recens is a high-frequency drug for NPDR treatment in traditional Chinese medicine. Its components may play a regulatory role on multiple pathways by acting on multiple targets, which can provide guidance for clinical and basic research.

DOI: 10.20047/j.issn1673-7210.2022.36.07
Renal fibrosis is the main pathological change of kidney disease progression to the later stage, and inflammatory stimulation is the main factor of renal fibrosis progression. Its irreversibility and outcome difference have become one of the difficult problems in modern medicine. The microRNA (miRNA) is a short-chain endogenous non-coding small molecule RNA present in stable form in plasma, serum and body fluids, post-transcriptionally regulated encoding protein genes, involved in the regulation of various biological processes. Studies have shown that some of the miR contained in miRNA play an important role in the fibrosis process and are key regulatory factors in the fibrosis/inflammation process. In kidney disease, miR-21 and miR-29 regulate target genes and related signaling pathways, acting on inflammation/promoting fibrosis process. This paper will focus on the signaling pathway of the pathogenesis of renal fibrosis infiltrated by inflammatory cells, and explore the mechanism and potential therapeutic effects of miR-21 and miR-29 in renal fibrosis from the molecular level.

DOI: 10.20047/j.issn1673-7210.2022.36.08
Essential hypertension is a common chronic non-communicable disease and is the most important risk factor for cardiovascular and cerebrovascular diseases. Currently, the main treatments for essential hypertension are pharmacological and non-pharmacological therapies. Clinically, although the patient’s blood pressure can be lowered by medication, the symptoms cannot be fully controlled and its toxic side effects are more frequent and add to the patient’s burden. Compared with drug therapy, the efficacy of lifestyle interventions and traditional Chinese medicine appropriate techniques such as acupuncture, moxibustion, massage, auricular pressure, acupuncture point application, scrapping therapy, ear tip bloodletting, herbal tea substitution, and traditional traditional Chinese medicine exercises such as taijiquan, baduanjin, and yijinjing are gradually being recognized both at home and abroad. This article reviews the progress in the application of non-pharmacological therapies for essential hypertension, with a view to providing a reference basis for the clinical application of non-pharmacological therapies for the treatment of hypertensive disorders.

DOI: 10.20047/j.issn1673-7210.2022.36.11

[Abstract] Chronic atrophic gastritis (CAG) is one of the common chronic diseases of the digestive system. It is complex and has a long course, and can be further developed to gastric cancer, which is a key link in the development process of gastric cancer. A variety of related inflammatory factors are involved in this link, and blocking this link is of great value to prevent the occurrence of gastric cancer. At present, modern medicine has no effective method to intervene in this link, so seeking effective intervention measures for CAG is the goal of many scholars. Studies have confirmed that traditional Chinese medicine has unique advantages in the treatment of CAG disease, and can exert potential effects through multi-directional, multi-level and multi-target. The purpose of this paper is to summarize the current situation and treatment prospect of traditional Chinese medicine in this field, and to provide basis for the future treatment of CAG, so as to better improve clinical efficacy.

122 Professor Sun Weizheng’s experience in treating chronic aplastic anemia in children
WU Yuantian1 SUN Weizheng2 YUSUN-Wanqi3 WANG Jinhuan2
DOI: 10.20047/j.issn1673-7210.2022.36.26
Aplastic anemia is a kind of hematological disease characterized by pancytopenia and decline of hematopoietic function of bone marrow, and its clinical manifestations are anemia, hemorrhage, repeated infection and so on. This paper introduces Professor Sun Weizheng’s clinical experience in the treatment of chronic aplastic anemia in children. Professor Sun believes that marrow depletion due to kidney deficiency is the core pathogenesis of the disease. Children’s delicate viscera, easy to transmit after feeling evil, and the physiological and pathological characteristics of being easy to injure yin make the performance of the disease in children more complex than that in adult patients. in addition, “blood stasis” and “heat” also have an important impact on the development and evolution of the disease. According to the etiology and pathogenesis of this disease, Professor Sun carefully examines yin and yang, distinguishes specimens, takes tonifying the kidney as the core, pays attention to taking care of children’s spleen and stomach, integrates the treatment of blood stasis and prevention of blood stasis in the whole process of treatment, and pays attention to the influence of daily life and emotions on the disease, and had excellent clinical effect.

131 Discussion on traditional Chinese medicine diagnosis and treatment of “stroke” based on the thinking of supporting yang and the ontology structure of yin and yang
XIE Rongxin1 WANG Jinping2 MO Xueni3
DOI: 10.20047/j.issn1673-7210.2022.36.28
Human life activities are completed by the interaction between yin and yang. In the position relationship between yin and yang, yang is relatively internal, and yin is relatively external. On the basis of this position, the upward outward movement of yang and the downward inward movement of yin realize the interaction between yin and yang. Therefore, it is believed that the core of traditional Chinese medicine diagnosis is to find the abnormal position of yin and yang or the abnormal sympathetic state under the internal yang and external yin structure of patients through pulse syndrome. The core of traditional Chinese medicine treatment is to restore the yin and yang of human body to the original position or normal sympathetic state, and the physical characteristics of stroke patients are summarized as “deficiency of yuan and yang” and “deficiency of yang” combined with clinical practice. The principle direction of warming and activating yang and the treatment method of “triple energizer secondary” are proposed.

Objective To use the data mining technology to explore and summarize the drug laws of the treatment of renal edema in Beiji Qianjin Yao Fang and Qianjin Yi Fang (hereinafter referred to as Qianjin Fang). Methods The prescriptions for treating renal edema in Qianjin Fang were selected to establish a standard medical record database, and the cloud platform of ancient and modern medical records was used to make statistics on drug frequency, drug properties, drug flavor, channel tropism, drug association rules, and cluster analysis. Results A total of 67 formulas for the treatment of renal edema from Qian Jin Fang were included in this study, involving 67 prescriptions, among which 44 were Beiji Qianjin Yao Fang and 23 were Qianjin Yi Fang. A total of 144 traditional Chinese medicine were involved, and the top three drugs with high frequency were Atractylodis Macrocephalae Rhizoma, Poria, and Cinnamomi Ramulus. The drug properties were more warm, the drug flavors were more sweet, pungent, and bitter, more return to the lung, spleen, kidney channel. The drug efficacy was more inducing diuresis for removing edema, invigorating spleen, diuresis and diffusing dampness, dispelling pathogenic wind and relieving pain, eliminating dampness and diuresis, and so on. In the association rules of the two drug patterns, the confidence of Stephaniae Tetrandrae Radix→Atractylodis Macrocephalae Rhizoma was the highest. Drugs used more than 20 times could be grouped into four groups by cluster analysis. Conclusion Through data mining, the differentiation and treatment ideas of renal edema in Qianjin Fang are shown, that is, the idea of taking deficiency and excess as the key line, sanjiao as the differentiation method and elimination as the method is formed, which provided reference for the clinical practice of traditional Chinese medicine treatment of renal edema.

DOI: 10.20047/j.issn1673-7210.2022.36.30
Chronic irritant dry cough is one of the most common symptoms of idiopathic interstitial pulmonary fibrosis (IPF). About 80% of IPF patients have chronic irritant dry cough, with daytime symptoms being the most common, and in severe cases it can lead to serious complications such as syncope and fractures, which can aggravate respiratory distress and hypoxia. However, the traditional cough medicine has poor curative effect, and the patients cough can not be cured, whith has a serious impact on patients’ quality of life and psychological well-being. Through clinical observation and practice, Professor Fan Maorong discusses the pathogenesis of cough associated with IPF from the perspective of “original deficiency and paralysis of the lung”, and treats it by emphasising the innate nature, the interplay of gold and water, nourishing the latter and mediating the middle jiao.

DOI: 10.20047/j.issn1673-7210.2022.36.31
Quanying Fanglun is written by Zheng Duanyou, a Southern Song Dynasty physician, as a monograph on paediatrics, the book is based on a collection of his clinical experience and the doctrines of various schools. Although it is not a monograph on moxibustion, it has a special volume on moxibustion for children. This paper analyzes the points for attention of children moxibustion method, moxibustion points for common diseases, and moxibustion epilepsy in the book. It is found that Quanying Fanglun advocates the same body inches method in the selection of acupoints, and believes that the selection of acupoints should be efficient and accurate. Precautions mainly involved in climate, patient status, time, location, moxibustion amount, and other aspects of avoidance. Among the 29 common diseases applicable to moxibustion method, most of them are liver and gallbladder diseases and heart diseases in internal medicine. The main points selected are conception channel, governor channel, and extra points. The book holds that the key to moxibustion epilepsy lies in deficiency and excess, and summarizes four syndroms of convulsive epilepsy. The moxibustion for children recorded in Quanying Fanglun still has a certain enlightening effect on pediatrics and clinical acupuncture and moxibustion, which has the value of in-depth exploration.

148 Professor Shao Yingxin’s experience in treating thyroid nodules is differentiated from qi deficiency
ZHENG Xiuyue1 SHAO Yingxin2
DOI: 10.20047/j.issn1673-7210.2022.36.32
The prevalence of thyroid nodules in Chinese adults is as high as 20%, western medicine mainly adopts surgical treatment and follow-up observation. Thyroid nodules are called gall tumor in traditional Chinese medicine. It is generally treated from the liver and spleen. It is considered that the liver is out of order due to emotional trauma, or the spleen is out of health and transportation due to improper diet, which leads to qi stagnation, phlegm coagulation and blood stasis, and obstructs the neck. The gall is treated by regulating qi and activating blood, resolving phlegm and eliminating gall. Professor Shao Yingxin believes that the main cause of this disease is congenital deficiency, namely fetal transmission, or ( and ) improper intake of iodine in the diet, and emotional but not the main cause. Due to congenital deficiency or acquired disorders caused by qi deficiency, qi deficiency can not promote the movement of qi, blood and body fluid is not smooth, resulting in the combination of qi, phlegm and blood stasis, the neck and the disease. The basic pathogenesis is qi deficiency, accumulation of phlegm and blood stasis, and the key pathogenesis is qi deficiency. Qi deficiency leads to qi stagnation, phlegm coagulation, blood stasis, and accumulation of phlegm and blood stasis, forming gall tumors. The basic syndrome type is deficiency of vital qi, phlegm and blood stasis syndrome. The treatment is tonifying qi and removing blood stasis, eliminating phlegm and dispersing stagnation. Clinically, it is proved that the treatment of gall tumors based on qi deficiency is satisfactory, which provides a new idea for the treatment of thyroid nodules and is worthy of reference and application.

DOI: 10.20047/j.issn1673-7210.2022.36.43
Objective To investigate and intervene the non-rational use of cephalosporins situation in medical orders of inpatients in the Second Hospital of Jinhua, Zhejiang Province (hereinafter referred to as “our hospital”). Methods  A total of 1 550 medical orders of cephalosporins issued to inpatients in the internal medicine ward of our hospital from January to December 2020 were selected for investigation, 750 medical orders from January to June 2020 were evaluated according to relevant standards, and 800 medical orders were intervened from July to December 2020. Relevant indicators in the doctor’s orders and the use of cephalosporins before and after intervention were counted, and the situation of non-rational use of drug before and after intervention was compared. Results Before and after intervention, the average number of drug varieties prescribed by doctors, the percentage of antibiotics prescribed by doctors, the proportion of cephalosporins used, the average amount of medical orders, and the proportion of cephalosporins prescribed by doctors were all in a stable and normal range. Most cephalosporins were Cefoperazone Sodium Sulbactam Sodium (41.94%), Ceftazidime (22.58%), Cefuroxime Sodium Powder Needle (9.67%), and Cefuroxime Ester Tablets (9.67%). After intervention, the incidence of unsuitable indications, unsuitable drug selection, unsuitable drug dosage form or route of administration, and unsuitable drug combination were all lower than those before intervention (P<0.05). The reasons for irrational drug use were mainly the lack of pharmacological knowledge of physicians and patients’ failure to follow the doctor’s advice. Conclusion Medical orders for cephalosporins in our hospital basically meet the standards, and non-rational use of drug is less. In the future, we should continue to strengthen the supervision of medical orders, conduct regular drug use investigation, and supervise rational drug use, so as to reduce the incidence of irrational drug use and provide guarantee for the safety of drug use for the majority of patients.
