Objective To explore the influencing factors of diabetic nephropathy (DN) in patients with type 2 diabetes mellitus (T2DM). Methods A total of 116 patients diagnosed with DN in Rugao People’s Hospital of Jiangsu Province (hereinafter referred to as “our hospital”) from September 2015 to December 2017 were selected and classified into DN group. In addition, 116 T2DM patients admitted to the Department of Endocrinology of our hospital during the same period were classified as T2DM group. The clinical baseline data of all patients were collected, and the level of platelet microparticles (PMPs) was detected by venous blood sampling. The differences of clinical indicators between two groups were analyzed, and the influencing factors of T2DM complicated with DN were analyzed. Results Diabetic course, glycosylated hemoglobin (HbA1c), urinary albumin excretion rate (UAER), creatinine (Cr), estimated glomerular filtration rate (eGFR), blood urea nitrogen (BUN) and PMPs levels in DN group were higher than those in T2DM group, while eGFR levels were lower than those in T2DM group (P < 0.05). Multivariate logistic regression analysis showed that HbA1c, UAER, and PMPs were independent risk factors for DN (OR > 1, P < 0.05). The area under the curve of combined diagnosis DN of UAER, HbA1c, and PMPs was higher than that of single detection of each index. Conclusion PMPs level is significantly increased in DN patients, HbA1c, UAER, and PMPs are the influencing factors of DN.

Objective To analyze the clinical characteristics and risk factors of neonatal refractory purulent meningitis. Methods The clinical data of 141 children with purulent meningitis who were admitted to the Department of Neonatology, Kunming Children’s Hospital from January to December 2017 were retrospectively analyzed. According to the bacterial culture results, they were divided into common group (99 cases) and refractory group (42 cases). General data, clinical manifestations, and laboratory test results of two groups were compared. The risk factors of refractory purulent meningitis in neonates were analyzed by logistic regression. Results There were no significant differences in the proportion of male infants, premature infant, birth weight < 2 500 g, amniotic fluid contamination, early-onset meningitis, and anti-infective treatment before admission between two groups (P > 0.05). There was no significant difference in the proportion of fever, convulsion, respiratory symptoms, and digestive tract symptoms between two groups (P > 0.05). The proportion of elevated blood plasma procalcitonin (PCT), positive bacteria culture was higher in refractory group than those in normal group, the white blood cell count and protein content of cerebrospinal fluid (CSF) were higher than those in normal group, and the glucose content of CSF was lower than that in normal group (P < 0.05); there was no significant difference in the proportion of abnormal peripheral blood white blood cell count and elevated blood C-reactive protein between two groups (P > 0.05). There was no significant difference in the distribution of pathogenic bacteria between two groups (P > 0.05). The total incidence of neurological symptoms between two groups was statistically significant (P < 0.05). The fever-reducing time and hospitalization time in refractory group were longer than those in normal group (P < 0.05); there was no significant difference in clinical outcome between two groups (P > 0.05). High CSF protein content, combined with neurological symptoms were independent risk factors for refractory neonatal purulent meningitis (OR > 1, P < 0.05), and high CSF glucose content was a protective factor for refractory neonatal purulent meningitis (OR < 1, P < 0.05). Conclusion The clinical manifestations of neonatal refractory purulent meningitis are not typical. The blood and CSF infection markers of the children are significantly abnormal, and neurological symptoms are prominent. Therefore, it is difficult to treat such children. High blood PCT and CSF protein content, and combined with neurological symptoms are independent risk factors for neonatal refractory purulent meningitis, and high CSF glucose content is a protective factor.

XU Run1 JIANG Quan2 HAN Man2 PENG Qiuwei2 FU Jingsi2 YUE Ming1
DOI: 10.20047/j.issn1673-7210.2022.24.27
This paper discusses Professor Jiang Quan’s experience in treating refractory gout based on the theory of “yang transforming qi, while yin shaping up body”. Professor Jiang Quan believes that the root of the pathogenesis of refractory gout is “the deficiency of yang transforming qi” in disorder of spleen and kidney gasification, and the “excessive yin shaping up body” in condensation of wet phlegm and stasis evil as the main pathological pathogenic factor. The treatment focuses on regulating the spleen and kidney, invigorating the spleen and invigorating qi, reducing the fire and vitality methods to help the gasification, and paying attention to life adjustment in order to protect the spleen and kidney. In addition, according to the course of the disease, clinical manifestations and it focuses on different methods of removing evil, based on syndrome differentiation to seek the cause, examining the cause to treat the cause. High uric acid focuses on promoting dampness and turbidity, while the acute attack focuses on clearing heat and promoting diuresis, removing edema and relieving pain, seeing gout stone increases the function of dissipating phlegm and resolving masses, and long-term arthralgia and blood stasis focuses on promoting blood circulation for removing blood stasis.

124 Professor Wang Qingguo’s experience in the treatment of alcoholic liver disease with Chaihu associated prescriptions
LIAO Xuejing ZHANG Shuang WANG Yifang KANG Xiangdong WANG Qingguo
DOI: 10.20047/j.issn1673-7210.2022.24.28
Alcoholic liver disease as a liver damage disease, due to excessive drinking, western medicine treatment has certain limitations. Professor Wang Qingguo has an outstanding effect on the treatment of alcoholic liver disease. He believes that alcoholic liver disease is root on stagnation of liver qi and spleen deficiency, and dampness and heat accumulation is the manifestation, which is also the initial pathogenesis. Therefore, the treatment should first take into account both manifestation and root cause of disease and focus on dispersing the stagnated liver-energy and invigorating the spleen, clearing heat and promoting diuresis throughout. Secondly, according to the law of pathogenesis evolution, stage combined with dialectical treatment. With the evolution of the disease, the pathogenesis shows stage changes, and in the middle stage, dampness and heat phlegm obstruction, stagnation of qi and blood stasis are most common. In the later stage, qi stagnation, blood stasis, phlegm and turbidity are intertwined. Until the dieases get worse, cause that dysfunction of liver, spleen and kidney, and qi, blood and body fluid are combined with each other. Therefore, in the middle and late treatment of alcoholic liver disease, according to the development of the pathogenesis of the disease, drugs such as promoting blood circulation for removing blood stasis, nourishing yin and resolving masses are used to strengthening the body resistance to eliminate pathogenic factors, and reinforcement and elimination in combination.

DOI: 10.20047/j.issn1673-7210.2022.24.41
IgG4-related sclerosing cholangitis (IgG4-SC) is a unique type of sclerosing cholangitis characterized by significant increase in serum IgG4 level, infiltration of IgG4-positive plasma cells and lymphocytes, fibrosis of bile duct wall and obliterans phlebitis. IgG4-SC is often complicated with autoimmune pancreatitis (AIP), which is sensitive to steroid hormones Due to the low incidence of IgG4-SC and the lack of specific clinical manifestations, it is easy to miss diagnosis and misdiagnosis, and the recurrence rate is high after treatment. This paper reports a case of IgG4-SC with jaundice as the first manifestation and without AIP who received steroid therapy in Liuzhou People’s Hospital of Guangxi Zhuang Autonomous Region in December 2018. No recurrence was found during the follow-up of two years and nine months. To discuss the definition, clinical characteristics and significance of typical cholangiography in the diagnosis and treatment of IgG4-SC, and the application value of serum IgG4 level in the diagnosis and treatment of IgG4-SC, so as to improve the diagnosis and treatment of IgG4-SC with jaundice as the first manifestation without AIP.

Intradural lumbar disc herniation (ILDH) is a rare disease with a high rate of misdiagnosis and missed diagnosis. As the herniated disc enters the dural, once misdiagnosed or missed diagnosis, the treatment effect may not be ideal, or even worse, causing huge psychological and economic burden to patients. In order to improve the clinical diagnosis and treatment level of this disease, the correct diagnosis and treatment plan was formulated. This paper analyzed and discussed a patient with ILDH admitted to the Department of Orthopedics of Hebei General People’s Hospital. The patient presented symptoms of nerve root involvement and cauda equina syndrome. Through preoperative physical examination and imaging data reading, the patient was considered as ILDH, posterior lumbar open surgery was performed to remove the nucleus pulposus tissue herniated into the dura, postoperative pathological examination confirmed ILDH, positive symptomatic treatment, good postoperative recovery. Based on the review and analysis of the domestic and foreign literature, most people believe that the diagnosis of ILDH should be actively treated by surgery; at the same time, literature review was carried out on the symptoms, clinical manifestations, imaging features and surgical methods of the disease to improve the understanding of the disease, so as to reduce misdiagnosis or missed diagnosis.

DOI: 10.20047/j.issn1673-7210.2022.24.46
As an important part of traditional Chinese medicine, genuine regional drugs not only have the characteristics of excellent quality and good effect, but also play a very important role in promoting regional economic development. At present, the traceability system of Chinese medicinal materials in China is still not perfect, and the traceability standards and norms are yet to be established, and the problem of information tampering cannot be fundamentally solved. In view of the problems of centralization, opacity, and easy tampering in traditional traceability methods, this paper takes the whole-process traceability of genuine regional drugs in Hunan Province as the starting point and uses blockchain technology to explore a quality traceability system of authentic medicinal materials with the characteristics of visualization, tamper-proof, traceability, decentralization, information openness, and transparency. It is expected to serve the high-quality development of the traditional Chinese medicine industry and set an example for consolidating targeted poverty alleviation and promoting rural revitalization.
