Study on small molecular metabolites in urine of school-age children with obesity#br#

Objective To analyze the changes of small molecular metabolites in urine of school-age children with obesity by gas chromatography-mass spectrometry (GC/MS) metabolomics technology, and screen out the differential metabolites in urine related to obesity in school-age children, so as to explore the occurrence mechanism of metabolic disorders in school-age children with obesity. Methods From March to December 2018, the list of physical examination schools in Department of Health Care, Baoshan District Hospital of Integrated Traditional Chinese Medicine and Western Medicine of Shanghai was numbered by random number table, and the physical examination data of children aged 12 to 15 in Baoshan Experimental Primary School, Shanghai University Affiliated Experimental School and Baoshan Gongfu Experimental School of Shanghai were randomly selected. Twenty-five obese children aged 12-15 were selected as the obese group, and a group of 25 normal children matched with age and sex were selected as the normal group. GC/MS was used to detect small molecular metabolites in urine of the two groups, and multidimensional statistical method-partial least squares-discriiminate (PLS-DA) was used to observe the urine metabolism profile of the two groups. According to the weight coefficient VIP>1, P < 0.05, the differential metabolites were obtained. Differential metabolites were identified using commercial metabolite profiles (NIST and Wiley Registry). Results Compared with normal group, height, body weight, body mass index, triglyceride, alanine aminotransferase, and low density lipoprotein were significantly increased in obese group, and the differences were statistically significant (P < 0.05 or P < 0.01), while high density lipoprotein was significantly decreased, and the difference was highly statistically significant (P < 0.01). The metabolic profiles of the obese group and the normal group were well separated and 38 different metabolites were identified. The area under the curve of receiver operating characteristic curve of 18 metabolites in urine, such as valine, isoleucine, and tyrosine, was > 0.5. School-age children with obesity have metabolic abnormalities such as phenylalanine, tyrosine and tryptophan biosynthesis, phenylalanine metabolism, glycerol metabolism, tyrosine metabolism and histidine metabolism. Conclusion Eighteen characteristic metabolites in urine such as valine, isoleucine, tyrosine and uric acid are expected to be potential biomarkers in the diagnosis of school-age children with obesity. Differential metabolites in urine of school-age children with obesity are screened by metabonomics method, which provide a certain experimental basis for the diagnosis and treatment of obesity in school-age children.

Visual analysis of the method of tonifying kidney in the treatment of dementia based on CiteSpace#br#

Objective To make a visual analysis of the method of tonifying kidney in the treatment of dementia based on CiteSpace. Methods Literatures related to the method of tonifying kidney in the treatment of dementia from the self-built database of China National Knowledge Infrastructure to October 1, 2021 were searched, Citespace software was used to make visual analysis on the authors, institutions, and keywords. Results Ni Jingnian from Dongzhimen Hospital of Beijing University of Chinese Medicine had the highest number of papers, and the authors in this field had formed multiple research teams; the affiliated institutions were mainly traditional Chinese medicine universities and their affiliated hospitals, and there was almost no connection between different institutions; the high-frequency key words include dementia, clinical research, tonifying kidney, animal model, traditional Chinese medicine, etc., the research hotspots in this field mainly focused on the mechanism research and clinical research of dementia. Conclusion Research hotspots mainly focus on drug action mechanism, clinical research, etc., oxidative stress, cognitive function, and medical record research are the current research trends, and there are few contacts between research institutions.

Visualization analysis of ulcerative colitis treated by traditional Chinese medicine based on CiteSpace#br#

Objective To analyze the research status and development trend of traditional Chinese medicine in the treatment of ulcerative colitis through the visualization software CiteSpace. Methods Related articles were retrieved from China National Knowledge Infrastructure, Wanfang, and VIP databases from January 2000 to October 2021, visualized analysis was conducted with authors, research institutions, and keywords of the articles through CiteSpace 5.8.R1. Results A total of 3135 articles were included, and the overall number of articles presented an upward trend; the core authors were Shen Hong, Wang Xinyue, and Tang Zhipeng; the main research institutions included Nanjing University of Chinese Medicine, Beijing University of Chinese Medicine, Longhua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine; the top three keywords were UC, retention enema, and traditional Chinese medicine therapy; the focus of domestic research was the use of UC in traditional Chinese medicine syndrome differentiation, the study of the clinical efficacy and mechanism exploration of traditional Chinese medicine in the treatment of UC. Conclusion The knowledge map directly shows the research status of traditional Chinese medicine treatment of UC in the past 21 years, the analysis shows that research will go further on the mechanism of action, and the signaling pathway, intestinal flora, and inflammatory factors are the research hot spots and trends in the future.

An empirical research on innovation efficiency of pharmaceutical enterprises based on stochastic frontier production function model#br#

Objective To study the current situation and influencing factors of innovation efficiency of pharmaceutical enterprises, and provide a scientific basis for improving the innovation efficiency of pharmaceutical enterprises. Methods The number of personnel and expenditures in pharmaceutical enterprises’ research and development were used as input indicators, and the number of patent applications was used as output indicators, the stochastic frontier production function model was used to calculate and compare the innovation efficiency of three types of pharmaceutical enterprises in chemical drug manufacturing, Chinese patent medicine manufacturing, and biological drug manufacturing from 2009 to 2019. Results The overall innovation efficiency of pharmaceutical enterprises showed a fluctuating upward trend. The elastic coefficients of the number of research and development personnel and the number of research and development expenditures were 25.285 (P < 0.01) and -9.164 (P < 0.01), there was a phenomenon of insufficient human investment and excess capital investment. The scale of enterprise innovation subject, the scale of research and development personnel, and the scale of research and development expenditure were the promoting factors of patent output efficiency, but the enthusiasm of enterprise research and development showed a restraining effect. Conclusion The overall innovation efficiency of pharmaceutical enterprises shows an upward trend, but there are problems such as insufficient research and development investment, low allocation efficiency, and uneven industry development. It is urgent for all relevant parties to increase the investment intensity of research and development resources in the pharmaceutical enterprises and optimize the allocation, improve industrial policies to achieve systematic management, and encourage pharmaceutical companies to change their operation and management thinking to improve management quality.

Effects of astragaloside Ⅳ-lipid polymerhybrid nanoparticles on rat model induced by ischemia-reperfusion injury#br#

Objective To investigate the effects of astragaloside Ⅳ-lipid polymerhybrid nanoparticles (AA-LPNs) on rat model induced by ischemia-reperfusion injury. Methods AA-LPNs was prepared by nano-precipitation method. Sixteen clean SD rats were selected and divided into the conventional group (8 rats) and the study group (8 rats) according to the random number table method. The conventional group was given astragaloside Ⅳ (AA) (40 mg/kg, 1.5 ml), and the study group was given AA-LPNs (40 mg/kg, 1.5 ml). The pharmacokinetic parameters of the two groups were compared (drug half-life [t1/2], peak time [Tmax], peak concentration [Cmax], area under concentration-time curve [AUC] [AUClast, AUCall], mean retention time [MRT] [MRTlast, MRTall]). Another 60 rats were selected, 15 of which were randomly selected as the sham operation group, and the rest were constructed as ischemia-reperfusion injury induced rat models. After successful modeling, the rats were divided into model group, AA group, and AA-LPNs group according to random number table method, with 15 rats in each group. Twenty four hours after modeling, the AA group and AA-LPNs group were given 40 mg/kg intragastric administration, while the sham operation group and model group were given 0.9% sodium chloride solution, once a day, for consecutive four weeks. After the experiment, the serum levels of creatine kinase (CK), lactate dehydrogenase (LDH), superoxide dismutase (SOD), malondialdehyde (MDA) in four groups were detected. TUNEL assay was used to detect the apoptosis of myocardial cells in the four groups. Results The AA-LPNs prepared in this study were spherical and uniformly round, with an average dynamic diameter of (180±23) nm. t1/2, Tmax, AUClast, AUCall, and MRTall in the study group were higher than those in the conventional group, and the differences were statistically significant (P < 0.05). Serum CK, LDH, MDA level and apoptosis rate in model group were higher than those in sham operation group, while SOD level was lower than that in sham operation group, the differences were highly statistically significant (P < 0.01). Serum CK, LDH, MDA level and apoptosis rate in AA group and AA-LPNs group were lower than those in model group, while SOD level was higher than that in model group, the differences were statistically significant (P < 0.05 or P < 0.01). Serum CK, LDH, MDA level and apoptosis rate in AA-LPNs group were lower than those in AA group, while SOD level was higher than that in AA group, the differences were statistically significant (P < 0.05 or P < 0.01). Conclusion AA-LPNs has a protective effect on myocardial cells in rats with ischemia-reperfusion injury.

Research on consumers’ willingness to use traditional Chinese medicine traceability system based on technology acceptance model#br#

Objective To analyze consumers’ willingness to use traditional Chinese medicine traceability system and its influencing factors. Methods A conceptual model of consumers’ willingness to use traditional Chinese medicine traceability system was established, stratified cluster random sampling was used to investigate eligible consumers, and structural equation model was used for factor analysis. Results The use attitude of traditional Chinese medicine traceability system significantly was affected by consumers’ health awareness, safety awareness of Chinese herbal medicine, perceived usefulness and perceived ease of use. The willingness to use of traditional Chinese medicine traceability system was positively affected by attitude to use. Pearson correlation analysis showed that perceived usefulness was positively correlated with perceived ease of use (r = 0.271, P < 0.01), and positively correlated with attitude of use (r = 0.129, P < 0.05). Perceived ease of use was positively correlated with health awareness (r = 0.142, P < 0.01), and health awareness was positively correlated with safety awareness of traditional Chinese medicine (r = 0.142, P < 0.01). There was a positive correlation between safety awareness and use attitude (r = 0.122, P < 0.05), and a positive correlation between use attitude and use willingness (r = 0.301, P < 0.01). Conclusion Concept empowerment to improve consumers’ cognition of traceable traditional Chinese medicine product attributes; technology empowerment to optimize the sense of experience and value of traceability information; the system and mechanism can enhance the credibility and influence of the traceability system.

Research progress of Chinese medicine in treating cough variant asthma in children#br#

Cough variant asthma (CVA) is a common and high incidence disease in pediatrics, and cough is the main clinical symptom of the disease. In clinical practice, it is often difficult to heal due to improper treatment, and finally evolve into bronchial asthma. At present, modern medicine has many side effects in the treatment of CVA in children, poor compliance of children and parents, and the disease is easy to occur repeatedly. Compared with Western medicine, traditional Chinese medicine, acupoint application, massage, acupuncture, integration of traditional and Western medicine and other methods have the characteristics of simple operation, less side effects, economic and practical, accurate curative effect and so on. With the gradual development of traditional Chinese medicine, the treatment of children’s CVA with traditional Chinese medicine has been accepted by more and more doctors and patients. This paper reviews the application progress of traditional Chinese medicine in the treatment of CVA in children.

Progress of effective parts and pharmacological action of Solanum lyratum Thunb.#br#

Solanum lyratum Thunb., a traditional Chinese medicine, is commonly used to treat cancer in clinic. Its effective parts mainly included aqueous extract, ethanol extracts, alkaloids, polysaccharides, and so on. Solanum lyratum Thunb. has significantly anticancer effect against hepatocellular carcinoma, lung cancer, gastric carcinoma, colorectal cancer, and ovarian cancer, also showed pharmacological action on immunomodulatory effect, antioxidation, antibacterial activity, and others. Based on the research achievements in the past five years, the effective parts and pharmacological actions of Solanum lyratum Thunb. are analyzed and summarized in this paper, while future research suggestions are put forward, in order to reflect the latest research development, to provide the theoretical basis for clinical application and reference for researchers’ next work.

Investigation on the current status and influencing factors of health literacy of general practitioners in the Yangtze River Delta region#br#

Objective To investigate and understand the current status of the health literacy level of general practitioners in the Yangtze River Delta region, to analyze its influencing factors, and to provide a basis for accurately improving the health literacy level of general practitioners. Methods From November to December 2020, a stratified multi-stage cluster sampling method was used to select general practitioners from 520 medical institutions in the Yangtze River Delta region (Shanghai, Jiangsu, Zhejiang and Anhui) to conduct a health literacy online questionnaire survey. The questionnaire consists of two parts, namely basic information and health literacy assessment (including basic theory, basic skills, healthy lifestyle and behavior, three dimensions). The scores of various dimensions of health literacy of general practitioners with different backgrounds were compared, and the influencing factors were analyzed. Results A total of 4531 valid questionnaires were recovered in this study, with an effective recovery rate of 90.52%. The basic theoretical literacy of general practitioners was (13.01±2.01) points, the basic skill literacy was (12.01±1.46) points, and the healthy lifestyle and behavioral literacy was (22.32±2.68) points. There were highly statistically significant differences in the health literacy scores of general practitioners in different provinces, genders, ages, educational backgrounds, professional titles, and working years (P < 0.01). The influencing factors of health theoretical literacy were gender (β=0.547), professional title (β=0.249), province (β=0.162) and educational background (β=0.13) in descending order; the main influencing factors of basic skills literacy were educational background (β=0.162) and working years (β=0.088); the main influencing factors of healthy lifestyle and behavior were gender (β=1.428), age (β=0.544), education (β=-0.196) and province (β=-0.156). Conclusion The health literacy of general practitioners in the Yangtze River Delta region of my country is relatively good, and they have the foundation to provide good health education for residents. However, there is still room for improvement in the dimensions of basic theoretical literacy and healthy lifestyle and behavior. Measures should be taken to improve the weak links general practitioners of health literacy.

Analysis of acquired immunodeficiency syndrome sentinel surveillance of key populations in Jingzhou from 2016 to 2020#br#

Objective To understand the acquired immunodeficiency syndrome (AIDS) epidemic trend and current situation of five key groups in Jingzhou from 2016 to 2020, and to provide clues for epidemic analysis and prevention. Methods According to the Requirements of the National AIDS Sentinel Monitoring Implementation Plan, a total of 14 000 people from five categories of drug users (DUS), men who had sex with men (MSM), female sex workers (FSW), male patients in clinics sexually transmitted disease (STD) and pregnant women (PRG) at the AIDS surveillance sites were collected in Jingzhou area from 2016 to 2020. While social demography, behavior and serological information of HIV, Syphilis, and hepatitis C virus (HCV) were performed. Results Most of DUS were married men, and the HIV positive detection rate had been well controlled during 2018 to 2020, but its hepatitis C infection rate was the highest. There was a statistically significant difference in the positive detection rates of HIV and syphilis in each year (P < 0.05). During the monitoring period from 2016 to 2020, the proportion of injecting drug users in the last month was 18% to 26%. MSM was dominated by young people with higher education, and its HIV infection rate was the highest among the population, with an upward trend during 2018 to 2020. There was a statistically significant difference in the positive detection rate of HCV in each year (P < 0.05). The proportion of anal sex with the same sex in the last six months reached 100% every year from 2016 to 2019. FSW was a married person with a low education level, the HIV infection rate was low, the syphilis detection rate was maintained at more than 2% every year, and the proportion of condom use in the last commercial sex during 2016 to 2020 had increased year by year, maintaining at 92.00% to 99.75%. STD covered all age groups. Due to the low proportion of prostitution in the past three months, the infection rates of HIV, syphilis and HCV were low. There was a statistically significant difference in the positive detection rate of syphilis in each year (P < 0.05). The positive detection rate of HIV, syphilis, and HCV in PRG was 0% during the monitoring period. There were statistical differences in age, education level, marital status, and positive detection rate of HIV, syphilis, and HCV among different groups (P < 0.05). The behavioral characteristics of each year were compared, and the differences were statistically significant (P < 0.05). Conclusion From 2016 to 2020, the HIV infection rate of MSM in Jingzhou is the highest and showed an upward trend. Syphilis is mainly transmitted in FSW, and DUS have the highest detection rate of HCV compared with other populations. The wide spread of HIV in MSM is related to its younger trend and high frequency of same-sex anal sex. It is necessary to strengthen the publicity and education of AIDS and its prevention and control knowledge among college students, and implement preventive intervention to curb the spread of AIDS.

Analysis of potential categories and influencing factors of hopelessness among rural empty-nest elderly#br#

Objective To explore the potential categories of the sense of hopelessness among the rural empty-nest elderly, and to analyze the differences in the characteristics of each category of rural empty-nest elderly. Methods From January to May 2021, a total of 400 empty-nest elderly people from 35 administrative villages in towns and towns under the jurisdiction of a city in Anhui Province were randomly clustered as the research objects, and the general data questionnaire and Beck hopelessness scale were used for investigation. Results The score of hopelessness among the rural empty-nest elderly was (8.60±3.86) points, and 51.7% of the elderly had a moderate to high degree of hopelessness. Hopelessness has three categories: high hopelessness-high motivation loss group (12%), moderate hopelessness-high negative anticipation group (61%), and low hopelessness group (27%). Compared with different marital status, monthly income, frequency of visiting children, frequency of going out for physical exercise, medical insurance, and sleep quality, and the differences were statistically significant (P < 0.05). Married and widowed (OR = 3.796), monthly income ≤ 1000 yuan (OR = 4.276), and never going out for physical exercise (OR = 3.390) were risk factors for the high incidence of hopelessness among the rural empty-nest elderly (P < 0.05); better sleep quality and new rural cooperative medical care were the protective factors for the high incidence of hopelessness (OR = 0.038, 0.116, P < 0.05). Conclusion The sense of hopelessness among the empty-nest elderly in rural areas has obvious classification characteristics, suggesting that it is necessary to find precise intervention targets from the perspective of active aging to alleviate the sense of hopelessness.

Difference analysis of serum 25-hydroxyvitamin D levels in children with tic disorder in Shanghai#br#

Objective To investigate the difference of serum 25- hydroxyvitamin D level in children with tic disorder (TD) in Shanghai. Methods From May 2018 to May 2020, 2001 children with TD aged 5 to 14 who visited the Department of Neurology of Shanghai Children’s Hospital (hereinafter referred to as “our hospital”) and 2670 normal children aged 5 to 14 who received physical examination in our hospital during the same period were selected. The serum 25-hydroxyvitamin D of children with TD was compared with that of normal children, and the serum 25-hydroxyvitamin D of children with TD with different characteristics was compared. Furthermore, children with TD were divided into five age groups according to one group every two years (24 months), including 5-6 years old group (655 cases), 7-8 years old group (237 cases), 9-10 old years group (610 cases), 11-12 years old group (342 cases), and 13-14 years old group (157 cases). The difference of 25- hydroxyvitamin D in the five groups was compared. Results The levels of serum 25-hydroxyvitamin D deficiency in children with TD was higher than those in normal children, the differences were statistically significant (P < 0.05). There were statistically significant differences in the levels of serum 25-hydroxyvitamin D deficiency among TD children with different residence, time of outdoor activities, personality of nuclear family members, eating habits, and study stress (P < 0.05). The levels of serum 25-hydroxyvitamin D deficiency in 9-10 years old group, 11-12 years old group, and 13-14 years old group was higher than those in 5-6 years old group and 7-8 years old group, the differences were statistically significant (P < 0.05). Conclusion There was a significant 25-hydroxyvitamin D deficiency in TD children in Shanghai, and the degree of 25-hydroxyvitamin D deficiency was different in TD children with different characteristics, and the deficiency was most significant in adolescents over nine years old.

Evaluation of drug-related problems and related factors in patients with chronic diseases#br#

Objective To study the current situation of drug related problems (DRPs) in patients with chronic diseases and analyze its influencing factors. Methods The medical records of 293 patients with chronic diseases were collected from Chongqing Dianjiang People’s Hospital from October 2020 to February 2021, and the incidence of DRPs was analyzed. According to the occurrence of DRPs, patients were divided into the occurrence group (132 cases) and the non-occurrence group (161 cases). The clinical data of the two groups were compared, and the influencing factors of the occurrence of DRPs in patients with chronic diseases were analyzed by binary logistic regression model. Results In this study, the incidence of DRPs was 45.05%, and a total of 165 DRPs were found. Bayliff evaluation results showed that DRPs caused six grade zero hazards, 133 grade one hazards, and 26 grade two hazards. There were statistically significant differences in age, number of complications, types of medication, and medication compliance between the two groups (P < 0.05). Multivariate analysis showed that age 60-75 years old, variety types of medication, number of complications, and poor medication compliance were independent risk factors for DRPs in patients with chronic diseases (OR > 1, P < 0.05). Conclusion Patients with chronic diseases have a high incidence of DRPs, and patients’ age, number of complications, medication varieties, and medication compliance all affect the risk of DRPs.

Investigation on the infection rate and related factors of toxocaracanis among preschoolers in Chongming District, Shanghai

Objective To evaluate the infection of toxocaracanis in preschool children in Chongming district, Shanghai, and analyze the related factors of toxocariasis infection. Methods The questionnaire survey was conducted among 1198 preschool children who studied in six kindergartens in Chongming District, Shanghai in 2018. The serum IgG antibody of toxocaracanis was detected in the children who returned valid questionnaires. The children were divided into infected group and uninfected group according to the detection results, and the influencing factors of toxocaracanis infection were analyzed. Results In this study, 1063 valid questionnaires were collected, with an effective rate of 88.73%. A total of 16 preschool children were positive for toxocariasis antibody, with a positive rate of 1.50%. The proportion of education about toxocaracanis, proportion of children who wash their hands regularly (after returning home, before meals, after using the toilet), and proportion of parents with bachelor degree or abovein infected group were all lower than those in non-infected group, and the differences were statistically significant (P < 0.05). Multifactor analysis showed that receiving education about toxocaracanis, children wash their hands regularly (after returning home, before meals, after using the toilet), and parents with bachelor degree or above were the protective factors of toxocaracanis infection in preschool children (OR < 1, P < 0.05). Conclusion The positive rate of serum toxocaracanis IgG antibody is low in preschool children in Chongming District, Shanghai, which is mainly due to the educational level of parents and good hygienic habits of children.

Clinical effects of Lixinshui Decoction combined with conventional Western medicine in the treatment of heart failure of coronary heart disease#br#

Objective To observe the clinical effects of Lixinshui Decoction combined with conventional Western medicine in the treatment of heart failure of coronary heart disease, and the effects of exercise tolerance, quality of life, N-terminal B-type natriuretic peptide (NT-proBNP), and vascular endothelial growth factor (VEGF) in patients. Methods A total of 82 coronary heart disease patients with heart failure admitted to Yancheng Hospital Affiliated to Nanjing University of Chinese Medicine from February to December 2020 were divided into control group and treatment group according to random number table method, with 40 patients in each group. Both groups were treated with conventional Western medicine as the basic treatment, the treatment group was treated with Lixinshui Decoction, the course of treatment in both groups was 12 weeks. Clinical efficacy, traditional Chinese medicine syndrome score, left ventricular ejection fraction (LVEF), Minnesota living with heart failure questionnaire (MLHFQ), 6-minute walking test (6MWT) distance, NT-proBNP, and VEGF were observed in both groups. The incidence of adverse reactions during treatment was recorded. Results There were two cases of detachment in the treatment group (one case of loss of follow-up, one case of gastrointestinal discomfort) and one case of detachment in the control group (loss of follow-up). A total of 79 patients were eventually included in this study. The clinical efficacy of the treatment group was better than that of the control group, and the difference was statistically significant (P < 0.05). After treatment, the traditional Chinese medicine syndromes scores in both groups were better than before, and the traditional Chinese medicine syndromes scores of shortness of breath, fear of cold and cold hand and foot, cyanotic lips and nails, malaise in the treatment group were better than those in the control group, and the differences were statistically significant (P < 0.05). After treatment, MLHFQ scores and NT-proBNP in both groups were lower than those before the treatment, 6MWT distance, LVEF, and VEGF were higher than those before the treatment, and MLHFQ and NT-proBNP in the treatment group were lower than those in the control group, 6MWT distance, LVEF, and VEGF were higher than those in the control group, the differences were statistically significant (P < 0.05). No adverse reactions occurred in the two groups during treatment. Conclusion Lixinshui Decoction combined with conventional Western medicine is effective in the treatment of heart failure of coronary heart disease, which can improve patients’ exercise tolerance and cardiac function, improve the quality of life, and promote angiogenesis.

Prediction research of the levels of plasma uric acid, atrial natriuretic peptide, and endothelin-1 with acute ST segment elevation myocardial infarction complicated with atrial fibrillation#br#

Objective To analyze the predictive value of the levels of plasma uric acid (UA), atrial natriuretic peptide (ANP), and endothelin-1 (ET-1) with acute ST segment elevation myocardial infarction (STEMI) complicated with atrial fibrillation. Methods A total of 156 newly diagnosed STEMI patients admitted to the Second Central Hospital of Baoding from January 2019 to may 2020 were selected for routine treatment and follow-up, and the incidence of atrial fibrillation was counted, and divided into concurrent group and non concurrent group according to whether complicated or not atrial fibrillation. The predictive value of the risk factors, each index alone, and in combination for patients with atrial fibrillation were analyzed. Results Age, body mass index (BMI), complication of ischemic cerebrovascular disease, complication of diabetes, complication of hypertension, complication of hyperlipidemia, left atrial diameter > 50 mm at initial diagnosis, cardiac function Killip classification>Ⅱ grade, newly diagnosed carbohydrate antigen (CA) 125 level, newly diagnosed UA level, newly diagnosed ANP level, newly diagnosed ET-1 level, and non-compliance with medical advice in the occurrence group were higher than those in the non-occurrence group (P < 0.05). Age, BMI, complication of ischemic cerebrovascular disease, complication of diabetes, complication of hypertension, complication of hyperlipidemia, left atrial diameter > 50 mm at initial diagnosis, cardiac function Killip classification>Ⅱ grade, newly diagnosed CA125 level, newly diagnosed UA level, newly diagnosed ANP level, newly diagnosed ET-1 level, and non-compliance with medical advice were all risk factors for patients with atrial fibrillation (R > 1, P < 0.05). Receiver operating characteristic curve showed that the area under the curve of joint prediction was higher than that of separate prediction (Z = 3.867, P = 0.001; Z = 2.838, P = 0.005; Z = 2.650, P = 0.008). Conclusion The plasma levels of UA, ANP, ET-1, and age, BMI are all risk factors of atrial fibrillation, and the combination of the three indicators are more effective in predicting atrial fibrillation than the separate indicators.

Effects of Sacubitril and Valsartan Sodium Tablets on ventricular remodeling in patients with heart failure complicated with hyponatremia#br#

Objective To investigate the effects of Sacubitril and Valsartan Sodium Tablets on ventricular remodeling in patients with heart failure complicated with hyponatremia. Methods A total of 82 patients with heart failure complicated with hyponatremia who were diagnosed and treated in Huai’an First People’s Hospital Branch from March 2017 to March 2020 were selected and divided into study group and control group by random number table method, with 41 cases in each group. The control group was given Tolvaptan Tablets, while the study group was given Tolvaptan Tablets, Sacubitril and Valsartan Sodium Tablets. The therapeutic effect and drug safety of the two groups were compared. Results The total effective rate of the study group was higher than that of the control group, and the difference was statistically significant (P < 0.05). After treatment, left ventricular ejection fraction (LVEF) in two groups was higher than before treatment, left ventricular end-diastolic diameter (LVEDD) and left ventricular end-systolic diameter (LVESD) were lower than those before treatment, and the differences were statistically significant (P < 0.05). After treatment, the LVEDD and LVESD in the study group were lower than those in the control group, while the LVEF was higher than that in the control group, with statistically significant differences (P < 0.05). After treatment, serum sodium in the two groups was higher than before treatment, and N-terminal brain natriuretic peptide precursor (NTpro-BNP) and interleukin-33 (IL-33) were lower than those before treatment, and the differences were statistically significant (P < 0.05). After treatment, the sodium content in the study group was higher than that in the control group, while the levels of NTpro-BNP and IL-33 in the study group were lower than those in the control group, and the differences were statistically significant (P < 0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Sacubitril and Valsartan Sodium Tablets in the treatment of heart failure complicated with hyponatremia can inhibit ventricular remodeling, improve the levels of hyponatremia, inflammatory factors, and neuroendocrine factors, thereby enhancing the therapeutic efficacy, with good safety.

Objective To investigate the qualitative study of the real experience of returning to work in young and middle-aged stroke patients by meta integration. Methods Web of Science, PsycINFO, Cochrane Library, CINHAL, Medline, PubMed, Embase, EBSCO, CNKI, SinoMed, Wanfang Data, VIP included qualitative studies on the return to work experience of young and middle-aged stroke patients. The retrieval time was from the establishment of the database to June 2021. The results were integrated using the quality evaluation criteria of Australian JBI evidence-based health care center and meta-integration method. Results A total of 15 literatures were included, and 60 research results were extracted, which were summarized into nine categoriesg and integrated into five integrated results. Strong willingness to return to work; complex emotional experience of returning to work; returning to work was affected by various factors; return to work, eager for help in many aspects; in the face of difficulties, actively seek change. Conclusion Young and middle-aged stroke patients face many challenges in the process of returning to work. To understand their feelings, experiences and needs in the process of returning to work can provide reference for the development of effective vocational rehabilitation programs.

Effect of transvaginal implantation of TiLOOP mesh in the treatment of anterior pelvic organ prolapse#br#

Objective To evaluate the short-term efficacy of transvaginal implantation of lightweight titanium polypropylene TiLOOP anterior pelvic floor repair system (TiLOOP Total 4) in the treatment of anterior pelvic organ prolapse. Methods Sixty patients with severe anterior pelvic organ prolapse who underwent anterior pelvic floor reconstruction in Putuo Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from April 2018 to May 2020 were selected as the research subjects. All patients underwent anterior vaginal wall mesh repair using TiLOOP Total 4. Perioperative indicators such as operative time, bleeding, incidence of postoperative diseases, and time to recover spontaneous urination were recorded; subjective satisfactive with surgery was assessed by patient global impression of change scale (PGI-C); pelvic organ prolapse quantitation (POP-Q) degree was used to evaluate the therapeutic effect. The reduction of indicator points was compared before surgery and 12 months after surgery; the quality of life of patients before surgery and 12 months after surgery was assessed by using pelvic floor impact questionnaire 7 (PFIQ-7); pelvic organ prolapse / urinary incontinence sexual function questionnaire short form 12 (PISQ-12) was used to evaluate the effect of surgery on sexual quality before surgery and 12 months after surgery. The occurrence of complications was recorded. Results All 60 patients were successfully operated without the occurrence of peripheral organ injury, pelvic vulvar hematoma or infection. The average operative time was (54.4±14.1) min, and the intraoperative blood loss was (65.2±22.5) ml. After surgery, two cases developed upper respiratory tract infection and one case developed urinary tract infection, which were cured after anti-infection treatment; the catheter was removed three days after surgery. One patient was found to have anterior vaginal wall bulge nine months after surgery, and the POP-Q degrees of the other 59 patients were ≤ Ⅰ degree. The PGI-C scores of 60 patients were ≥4 points. The midline of the anterior wall of the vagina 3 cm from the hymen (Aa), the furthest point in the upper segment of the anterior wall between the apex of the vagina or anterior fornix and Aa, and the furthest point of the apex of the vagina after cervical or hysterectomy were significantly improved 12 months after surgery, with highly statistically significant differences (P < 0.01). There were highly significantly differents in PFIQ-7 total score and the simple list scores before surgery and 12 months after surgery (P < 0.01). A total of 28 patients resumed sexual life three months after surgery, there were highly significantly differents in PISQ-12 scores before surgery and 12 months after surgery (P < 0.01). Up to 12 months after surgery, the incidence of vaginal anterior wall mesh exposure was 5%, and the incidence of mild acid distension pain in thigh root and lumbosacral region was 6.7%. Conclusion TiLOOP Total 4 has positive short-term efficacy in patients with anterior pelvic organ prolapse, with fewer postoperative complications, and the long-term efficacy needs further follow-up.

Visualization analysis of tic disorder in children based on CiteSpace and VOSviewer#br#

Objective To study tic disorder in children during the past 10 years in China using visualization analysis, and to understand the research status, strengths and weaknesses of tic disorder in children in China. Methods The three databases of Wanfang, CNKI, and VIP were searched, and the literatures meeting the research requirements were selected from January 2010 to December 2019. Keywords co-occurrence, author collaboration, and emergence word analysis were performed by CiteSpace and VOSviewer. Results A total of 2083 domestic literatures on tic disorder in children were screened out in the past ten years, the authors with frequency > 5 were cooperative analyzed, and 8 clustering teams were obtained; hot words that still had influence in 2019 include “psychological intervention”, “acupuncture therapy”, “auricular acupoint sticking”, etc.; three large cluster analyses were obtained by co-occurrence analysis of keywords with frequency > 10. Conclusion Domestic research on tic disorder in children is in a good development trend, and research progress has been made in the “pathogenesis”, “traditional Chinese medicine treatment”, “drug therapy”, “psychological therapy”, and other fields. There is cooperation among many authors in China, but the cooperation between teams is not close. At present, there are few researches on tic disorders in children at the cellular, molecular and genetic levels in China. In the future, more in-depth exploration of its pathogenesis and treatment is needed to better prevent and treat tic disorder in children.

Severe aplastic anemia and very severe aplastic anemia are critically ill, and the rate of implantation failure and transplant-related mortality will increase if the patients are associated with severe infection before transplantation. This article introduces the infection types, anti-infection programs, transplantation timing, pretreatment, graft-versus-host disease prevention programs and implantation situation of three children with severe infection before hematopoietic stem cell transplantation in Beijing Children’s Hospital, Capital Medical University. It can be seen from the prognosis of children that co-infection is not contraindicated for transplantation, anti-infection treatment should be actively carried out before transplantation, according to the guidelines of neutropenia with fever, the patient should be transplanted as soon as possible after infection control is stable, and salvage transplantation is feasible when it is difficult to control.

Changes and diagnostic value of C-reactive protein / albumin, Toll-like receptor 4, procalcitonin, and soluble intercellular adhesion molecule 1 levels in children with complex appendicitis#br#

Objective To research the changes and diagnostic value of C-reactive protein/albumin (CRP/ALB), Toll-like receptor 4 (TLR-4), procalcitonin (PCT), and soluble intercellular adhesion molecule 1 (sICAM-1) levels in children with complex appendicitis. Methods A total of 86 children with acute appendicitis who underwent surgical treatment in Anhui Children’s Hospital from June 2018 to June 2021 were selected, and they were divided into simple appendicitis group (34 cases) and complex appendicitis group (52 cases) according to the pathological type. The levels of CRP/ALB, PCT, TLR-4, and sICAM-1 in two groups were compared, the indexes affecting complex appendicitis were analyzed by logistic regression, the receiver operating characteristic (ROC) curve of each index was drawn, and the area under curve (AUC), sensitivity, and specificity were calculated. Results The levels of CRP/ALB, TLR-4, PCT, and sICAM-1 in complex appendicitis group were higher than those in simple appendicitis group, and the differences were statistically significant (P < 0.05). Logistic regression analysis showed that CRP/ALB, TLR-4, PCT, and sICAM-1 were the risk factors of complex appendicitis (OR > 1, P < 0.05). The AUC of four combined test in predicting complex appendicitis was higher than that of CRP/ALB, TLR-4, PCT, and sICAM-1 alone (P < 0.05); the sensitivity and specificity of four combined test were higher than that of CRP/ALb, TLR-4, PCT, and sICAM-1 alone. Conclusion When complex appendicitis occurs, the levels of CRP/ALB, TLR-4, PCT, and sICAM-1 in children are abnormally elevated, which are risk factors affecting complex appendicitis and have certain predictive value in prediction of appendicitis. The combined detection of the four indicators has good diagnostic and pathological value for complex appendicitis.

Effects of external radiotherapy combined with endocrine therapy on patients with biochemical recurrence after radical prostatectomy#br#

Objective To evaluate the effects of external radiotherapy combined with endocrine therapy on patients with biochemical recurrence after radical prostatectomy. Methods A total of 132 patients with biochemical recurrence after radical prostatic cancer surgery in Department of Urology, Xuzhou Central Hospital of Jiangsu Province from January 2010 to December 2018 were selected. According to the different treatment regimens after biochemical recurrence, the patients were divided into three groups: external radiotherapy group (36 cases), endocrine therapy group (45 cases), and combined treatment group (external radiotherapy combined with endocrine therapy, 51 cases). The clinical data of the three groups were collected and compared, and the survival and adverse reactions of the three groups were compared after follow-up. Results There were no significant differences in age, body mass index, preoperative prostate specific antigen (PSA), Gleason score, clinical stage, and PSA doubling time among the three groups (P > 0.05). All patients were followed up for 24 to 132 months, with a median follow-up of 63.5 months. Biochemical recurence-free survival rates were 50.0%, 51.1% and 66.7% in the external radiotherapy group, the endocrine therapy group, and the combination treatment group, with median survival rates of 62.0, 67.9, and 78.1 months, respectively. The biochemical recurence-free survival rates in the combination treatment group were higher than those in the external radiotherapy group and the endocrine therapy group, the difference was statistically significant (P < 0.05). There were no significant differences in the incidence of hot flashes, urethral reactions, intestinal reactions, and sexual dysfunction among the three groups (P > 0.05). Conclusion For patients with biochemical recurrence after radical prostatectomy, the combination of external radiotherapy and endocrine therapy group had more advantages in prolonging median survival time, and there was no significant difference in adverse reactions. Overall, the balance between survival benefit and treatment safety was better achieved.

Effect of 5-fluorouracil blood concentration monitoring on individualized treatment in patients with gastrointestinal tumors#br#

Objective To investigate the effect of 5-fluorouracil (5-Fu) blood concentration monitoring on individualized treatment in patients with gastrointestinal tumors. Methods A total of 54 patients with gastrointestinal tumors who were admitted to Zhongda Hospital Affiliated to Southeast University from November 2019 to August 2020 were selected as the research objects, they were divided into control group and study group by random number table method, with 27 cases in each group. Both groups were treated with FOLFOX6 regimen for chemotherapy. In the first cycle of chemotherapy, both groups were given the traditional body surface area (BSA) based chemotherapy. From the second cycle to the end of chemotherapy, the control group were still treated with BSA. In the study group, the dose of drug was adjusted from the second cycle to the end of chemotherapy based on the plasma drug concentration and pharmacological parameter(area under the curve) of 5-FU in the first cycle of chemotherapy. The distribution of 5-Fu plasma drug concentration, chemotherapy effect and adverse reactions between the two groups in the fourth cycle of chemotherapy were compared. Results There were significant difference in the proportion of 5-Fu plasma drug concentration distribution between the two groups (P < 0.05). After four cycles of chemotherapy, the overall response rate of the study group was significantly higher than that of the control group (P < 0.05). There was no significant difference in the incidence of grade Ⅰ-Ⅱ bone marrow suppression, and hand-foot syndrome, grade Ⅲ-Ⅳ mucositis, and hand-foot syndrome between the two groups (P > 0.05); The incidence of grade Ⅰ-Ⅱ diarrhea, gastrointestinal reaction, mucositis, and grade Ⅲ-Ⅳ diarrhea, gastrointestinal reaction, and bone marrow suppression in the study group was significantly lower than that in the control group, and the difference was statistically significant (P < 0.05). Conclusion Monitoring the plasma drug concentration of 5-Fu in patients with gastrointestinal tumors can provide dosage basis for individualized treatment, improve the chemotherapy effect of individualized treatment and reduce the incidence of adverse reactions.

Objective To investigate the relationship between ROS1 gene and efficiency of pemetrexed combined with platinum chemotherapy in patients with stage Ⅳ lung adenocarcinoma. Methods A total of 384 patients with stage Ⅳ lung adenocarcinoma treated in the Tumor Hospital Affiliated to Xinjiang Medical University from June 2015 to September 2018 were collected. According to ROS1 gene status, patients were divided into ROS1 positive group (32 cases) and ROS1 negative group (352 cases). All patients underwent ROS1 gene detection and received Pemetrexed combined with platinum chemotherapy. Relationships between objetive reposnse rate (ORR), disease control rate (DCR), progression free survival (PFS) and ROS1 gene status were analyzed. Results The ORR, DCR of ROS1 positive group were higher than ROS1 negative group (P < 0.05). The survival rate of patients in ROS1 positive group was higher than that in ROS1 negative group (P < 0.05). ROS1 gene was an influencing factor of death (HR = 0.590, 95%CI:0.403-0.863, P < 0.05). Conclusion The first-line treatment of ROS1 positive patients with stage Ⅳ lung adenocarcinoma with Pemetrexed combined with platinum chemotherapy is better, and the time without disease progression is prolonged.

Study on the proton and heavy ion radiotherapy tumor patients of social support status and influencing factors#br#

Objective To investigate the social support status and analyze the influence factors of tumor patients who has received radiotherapy in Shanghai Proton and Heavy Ion Center (“our hospital” for short). Methods A total of 151 tumor patients who received proton and heavy ion radiotherapy in our hospital from July 2020 to March 2021 were selected by convenient sampling method. The general information and social support status of patients were investigated by self-made questionnaire and social support scale, and the social support status and influencing factors of tumor patients were analyzed. Results The tumor patients with proton heavy ion radiotherapy were in the middle level of social support, which was highly statistically significant compared with the norm (P < 0.01). There were significant differences in social support among patients of different ages, educational level, average monthly household income and the location of tumor (P < 0.05). Logistic regression analysis showed that pelvic tumor was a protective factor for low and high level of social support (OR = 0.208, 95%CI=0.064-0.672), and married and have children was a risk factor for low and high level of social support (OR = 10.483, 95%CI=1.279-85.887). Conclusion The overall social support status of tumor patients treated with proton heavy ion radiotherapy is good. During diagnosis and treatment, medical staff should establish effective communication channels, carry out personalized diagnosis, treatment and nursing of patients, and strengthen the education of family members, so as to improve the social support status of patients and give play to the role of social support in promoting health.

They were divided into the observation group and the control group by random lottery method, with 48 cases in each group. The control group was treated with conventional ESD, and the observation group was treated with titanium clip combined with nylon rope traction on the basis of the control group. The perioperative indexes of the two groups were observed. Inflammatory response and stress response indexes (interleukin-6 [IL-6], tumor necrosis factor -α [TNF-α], epinephrine [E], norepinephrine [NE]), genetic molecules [transforming growth factor-α [TGF-α], cyclin [cyclin C]) were determined before and after surgery and postoperative complications between the two groups were compared. Results The operative time in the observation group was short than the control group, times of submucosal supplementary injection was less than the control group, intraoperative gastric perforation and intraoperative bleeding were lower than those in the control group (P < 0.05). Three days after operation the levels of IL-6, TNF-α, NE, and E in the two groups were higher than before operation, and the observation group was lower than the control group (P < 0.05). The overall analysis showed that there were statistically significant differences between groups, time points and interactions (P < 0.05). Intra-group comparison: levels of TGF-α and cyclin E in both groups were significantly higher than before operation one week and one month after operation, and one month after operation was lower than one week after operation (P < 0.05). Comparison between groups: there was no significant difference in TGF-α and cyclin E between the two groups before operation (P > 0.05). The levels of TGF-α and cyclin E in the observation group were significantly lower than those in the control group at one week and one month after operation. There was no significant difference in postoperative complication rate between the two groups (P > 0.05). Conclusion Titanium clip combined with nylon rope traction in ESD of early angular gastric cancer can significantly reduce the operation time and intraoperative bleeding, which has clinical promotion value.

Expression and clinical significance of LncRNA LINC00355, miR-494-3p, and CCND2 mRNA in prostate cancer tissues#br#

Objective To investigate the expression and clinical significance of long non-coding RNA (LncRNA) LINC00355, microRNA-494-3p (miRNA-494-3p), cyclin D2 (CCND2) mRNA expression in prostate cancer tissues. Methods A total of 91 prostate cancer patients admitted to Anhui NO.2 Provincial People’s Hospital from January 2015 to December 2017 were selected. qRT-PCR was used to detect the expression of LncRNA LINC00355, miR-494-3p, and CCND2 mRNA in cancer tissues and paracancer normal tissues of patients. Kaplan-Meier curve was used to analyze three-year survival rate after surgery of prostate cancers with different LncRNA LINC00355, miR-494-3p, and CCND2 mRNA expression. Results LncRNA LINC00355 and CCND2 mRNA expression in prostate cancer tissues were higher than those in paracancer normal tissues, and miR-494-3p expression was lower than that in paracancer normal tissues (P < 0.05). LncRNA LINC00355 was negatively correlated with miR-494-3p expression and positively correlated with CCND2 mRNA expression in prostate cancer tissues (r = -0.502, 0.458, P < 0.001), and miR-494-3p was negatively correlated with CCND2 mRNA expression (r = -0.493, P < 0.001). The three-year overall survival rate after surgery in patients with high LncRNA LINC00355 expression, low miR-494-3p expression, and high CCND2 mRNA expression were lower than those in patients with low LncRNA LINC00355 expression, high miR-494-3p expression, and low CCND2 mRNA expression (P < 0.05). Conclusion LncRNA LINC00355 and CCND2 expression are up-regulated and miR-494-3p expression is down-regulated in prostate cancer tissues. LncRNA LINC00355 may promote prostate cancer progression by inhibiting miR-494-3p and up-regulating CCND2 expression.

Clinical study of modified PCH regimen for locally advanced breast cancer with positive human epidermal growth factor receptor 2 #br#

Objective To study the clinical efficacy of modified PCH regimen (Trastuzumab+Paclitaxel+Carboplatin) for locally advanced female breast cancer with positive human epidermal growth factor receptor 2 (HER-2). Methods A total of 134 patients with locally advanced female breast cancer with positive HER-2 admitted to Binhai People’s Hospital Affiliated to Kangda College of Nanjing Medical University were selected, they were divided into observation group and control group by random number table method, with 67 cases in each group. The control group received Paclitaxel and Carboplatin, and the observation group received modified PCH regimen. After chemotherapy, both groups received modified radical mastectomy for breast cancer. The clinical efficacy of two groups was observed; serum tissue polypeptide specific antigen (TPS) and carcinoembryonic antigen (CEA), carbohydrate antigen 125 (CA125), CD3+, CD4+, CD8+, and CD4+/CD8+ were detected before treatment and six months after treatment; the incidence of adverse reactions in two groups was recorded. Results The disease control rate in observation group was higher than that in control group (P < 0.05). Six months after treatment, the levels of TPS, CEA, and CA125 in both groups were lower than those before treatment, and observation group was lower than control group (P < 0.05). Six months after treatment, CD3+, CD4+, and CD4+/CD8+ in both groups were decreased compared with before treatment (P < 0.05); there was no significant difference in CD8+ levels between two groups (P > 0.05). Six months after treatment, there were no significant differences in CD3+, CD4+, CD8+, and CD4+/CD8+ between two groups (P > 0.05). The total incidence of adverse reactions in observation group was lower than that in control group (P < 0.05). Conclusion Modified PCH regimen has a good efficacy for locally advanced female breast cancer with positive HER-2, which can reduce serum tumor marker level and is not likely to damage immune function, with high safety.

Relationship between interleukin, adiponectin gene polymorphism and breast cancer#br#

Objective To explore the relationship between interleukin (IL), adiponectin (APN) gene polymorphisms and breast cancer Methods This study was a retrospective analysis, a total of 96 patients with breast cancer admitted to Rugao Hospital Affiliated to Nantong University (“our hospital” for short) from January 2018 to January 2020 were divided into breast cancer group, another 96 healthy women in our hospital who received physical examination in the same period were selected as normal control group. The general data of two groups were collected, and the gene polymorphisms of IL and APN genes were detected, the influencing factors of female breast cancer were analyzed. Results There were statistically significant differences between two groups in IL-10 gene promoter region-1082 genotype, IL-16 gene rs1556218 genotype, IL-6 gene-572 genotype, IL-8 gene-251 genotype, and APN gene-276 genotype (P < 0.05). IL-10 gene promoter region -1082 GA genotype, IL-16 gene rs1556218 TG genotype, IL-6 gene-572 CG genotype, IL-8 gene-251 TA genotype, and APN gene- 276TG genotype were influencing factors of female breast cancer (P < 0.05). Conclusion The occurrence of female breast cancer is closely related to IL-10 gene promoter region-1082 GA genotype, IL-16 gene rs1556218 TG genotype, IL-6 gene-572 CG genotype, IL-8 gene-251 TA genotype, and APN gene-276 TG genotype.

Effects of different body positions on painless bronchoscopy and influencing factors of operation time#br#

Objective To explore the effects of different positions on painless bronchoscopy and the influencing factors of operation time. Methods Patients who underwent bronchoscopy from Digestive Endoscopy Center of the First Affiliated Hospital of Wenzhou Medical University from March to May 2019 were selected as the research subjects. According to the position of the patients during the operation, they were divided into the head back position group and the pillow-free supine position group, and the angle between the neck and the mandible of each patient was measured. The blood pressure, heart rate, blood oxygen saturation; the success rate of bronchoscopy into the glottis, and the operation time; the reaction of coughs and the injury of the nasopharyngeal mucosa were compared between the two groups. At the same time, multiple linear regression was used to analyze the influencing factors of the operation time of bronchoscopy into the glottis. Results In the end, a total of 200 patients were included in this study, including 103 patients in the head back position group and 97 patients in the supine position without the pillow. There was no significant difference in operation time between the two groups (P > 0.05). The successful rate of entering the glottis in the head back position group was higher than that in the pillow-free position group, and the differences were highly statistically significant (P < 0.01). There was no significant difference in the reaction of choking coughs and the injury rate of nasopharyngeal mucosa between the two groups (P > 0.05). The results of regression analysis showed that the mandible and the angle between the neck was closely related to the operation time (P < 0.05). Conclusion The success rate of bronchoscopy into the glottis is higher in the head back position group than in the supine position without the pillow. The angle between the neck and the mandible is an important factor affecting the operation time of painless bronchoscopy into the glottis.

Influencing factors of resuscitation time in patients undergoing ileal bladder replacement surgery for bladder cancer#br#

Objective To analyze the influencing factors of resuscitation time in patients undergoing ileal bladder replacement surgery for bladder cancer. Methods A retrospective analysis was performed on the clinical data of 408 patients with bladder cancer who underwent ileal bladder replacement surgery in West China Hospital of Sichuan University from March 2018 to December 2020. The data of all patients were complete, the resuscitation time of all patients was counted, and the baseline data filling form was designed to fill in the baseline data of patients in detail. The resuscitation time of patients resuscitated after ileal bladder replacement surgery for bladder cancer was compared with different data characteristics, and the influencing factors of resuscitation time of patients resuscitated after ileal bladder replacement surgery for bladder cancer were analyzed. Results The resuscitation time of 408 patients with bladder cancer after ileal bladder replacement was (65.19±8.33) min. The resuscitation time of patients with intraoperative hypothermia was longer than that of patients without intraoperative hypothermia, the difference was statistically significant (P < 0.05). Comparison of other data between patients with intraoperative hypothermia and patients without intraoperative hypothermia showed no statistical significance (P > 0.05). The average operation time of 408 patients was (4.83±1.10) h; the average intraoperative dosage of propofol was (135.31±20.15) ml, and the average intraoperative infusion volume was (3365.38±235.41) ml. Multiple linear regression analysis showed that long operation time (X1), intraoperative hypothermia (X2), large amount of propofol (X3) and large amount of infusion (X4) were the influencing factors for the resuscitation time of patients undergoing ileal bladder replacement surgery for bladder cancer (P < 0.05). The equation was obtained as Y = 97.809+ 2.820X1-2.141X2 + 0.005X3 + 0.007X4. The F value of the regression model was 31.724, R2 was 0.239, and R2 after adjustment was 0.232. Conclusion The resuscitation time of patients undergoing ileal bladder replacement surgery for bladder cancer is longer, which may be related to the operation time, intraoperative propofol dosage, intraoperative infusion volume and intraoperative hypothermia.

Study on medication rule of Professor Liu Lifang in treating upper limb lymphedema of breast cancer#br#

Objective To summarize the medication rule of Professor Liu Lifang in treating upper limb lymphedema of breast cancer. Methods Clinical diagnosis and treatment prescriptions of Professor Liu in treating upper limb lymphedema of breast cancer in the First Hospital of Hunan University of Chinese Medicine from July 2018 to February 2019 were summarized and corrected, then the dispatch medication database was established. The database was deeply mined by using software. Medication frequency, medication mode and compatibility were statistically analyzed. Results A total of 71 prescriptions were included, with a total frequency of 1593 times, 58 drugs and 16 core drugs. The frequently used drugs were diuretic dampness excreting drugs, tonic drugs, blood-activating and stasis-resolving drugs and warm yang drugs. The drug meridians were mainly distributed in spleen, liver, lung, kidney, stomach and heart meridians. The drug properties were mainly warm, cold, and flat, and the drug taste was mainly sweet, bitter, and pungent. The top five drugs with the most frequent occurrence were Poria, Atractylodis Macrocephalae Rhizoma, Bupleuri Radix, Astragali Radix, and Angelicae Sinensis Radix. The most common drug combinations were Poria-Atractylodis Macrocephalae Rhizoma, Poria-Astragali Radix, and Poria-Bupleuri Radix. Six core drug combinations and three new prescriptions were obtained by cluster analysis. Conclusion Professor Liu in treating upper limb lymphedema of breast cancer is based on syndrome differentiation and treatment. The drugs used are mainly diuretic dampness excreting drugs, tonic drugs, blood-activating and stasis-resolving drugs and warm yang drugs. The treatment is based on promoting water and dampness, warming and tonifying spleen and kidney, supplemented by supplementing qi and strengthening spleen, regulating and tonifying spleen and lung, promoting blood circulation and removing blood stasis, etc., to realize the principle of “combination of disease and syndrome” in the whole process.

Through years of clinical observation and practice, Professor Huang Shijing believes that the key pathogenesis of senile insomnia is “spinal cord reduced cerebral council due to kidney deficiency”. And around this pathogenesis, he puts forward the basic treatment of “tonifying the kidney to reinforce original qi, nourishing brain essence to tranquilize spirit”. In clinical treatment, it should be used throughout the treatment process, and supplemented by two medical cases to provide some ideas and methods for clinical diagnosis and treatment.

Objective To analyze medication rule of traditional Chinese medicine in treating pancreatic cancer based on traditional Chinese medicine inheritance computer system. Methods The literatures related to traditional Chinese medicine in treating pancreatic cancer in CNKI, Wanfang database, VIP and China biomedical literature database from inception to May 2021 were retrieved. Medication rule of traditional Chinese medicine in treating pancreatic cancer was summarized by using traditional Chinese medicine inheritance computer system. Results A total of 56 prescriptions and 178 Chinese herbs were included. The most commonly used traditional Chinese medicines were Poria, Atractylodis Macrocephalae Rhizoma, Astragali Radix, Pinelliae Rhizoma, Hedyotis Herba, Paeoniae Radix Alba and Coicis Semen. The property of traditional Chinese medicine was mainly cold; the medicine mainly tasted bitter, sweet, and pungent; drugs mainly returned to the spleen and liver channels; the main drug effects were tonifying deficiency, promoting water and dampness, and clearing away heat. Four core drug combinations were obtained by cluster analysis. Conclusion Traditional Chinese medicine in treating pancreatic cancer is based on the principle of invigorating spleen and eliminating dampness, while taking into account soothing liver and dispersing stagnation, promoting blood circulation and removing blood stasis.

Effect of thunder-fire moxibustion in treating sore throat after tracheal intubation under general anesthesia#br#

Objective To investigate clinical effect of thunder-fire moxibustion in treating sore throat after tracheal intubation under general anesthesia. Methods One hundred and eighty patients with sore throat after laparoscopic tracheal intubation under general anesthesia at Department of Gynecology in the First Affiliated Hospital of Guangxi University of Science and Technology from July 2019 to May 2021 were selected and divided into two groups according to random number table method, with 90 cases in each group. Thunder-fire moxibustion was used in observation group, Ambroxol Hydrochloride atomization inhalation was applied in control group. The scores of Bergman comfort scale (BCS) at 12, 24, 48 h, and 72 h after operation, the effect of pharyngeal foreign body sensation and patient satisfaction at 72 h after operation were compared between two groups. Results The overall analysis showed that there were significant differences in BCS score between groups, time points and interaction (P < 0.05). Intra group comparison: BCS scores of two groups at 24, 48, and 72 h after operation were higher than those at 12 h after operation (P < 0.05), BCS scores of two groups at 48, and 72 h after operation were higher than those at 24 h after operation (P < 0.05), BCS scores of two groups at 72 h after operation were higher than those at 48 h after operation (P < 0.05). Comparison between groups: BCS scores of observation group at 24, 48, and 72 h after operation were higher than those of control group (P < 0.05). The effect of pharyngeal foreign body sensation in observation group was better than that in control group 72 h after operation, and the difference was statistically significant (P < 0.05). The satisfaction of observation group 72 h after operation was better than that of control group, the difference was statistically significant (P < 0.05). Conclusion Thunder-fire moxibustion can effectively reduce the symptom of sore throat after tracheal intubation under general anesthesia, eliminate the feeling of foreign body in pharynx, and improve the treatment satisfaction of patient.

Clinical effect of acupuncture at shéntáng and nǎohù points on post-stroke depression#br#

Objective To observe the clinical effect of acupuncture at shéntáng and nǎohù points on post-stroke depression (PSD). Methods A total of 60 PSD patients admitted to the Department of Rehabilitation, Shijiazhuang Hospital of Traditional Chinese Medicine from October 2018 to December 2020 were selected and they were divided into the control group and the treatment group by random number table method, with 30 patients in each group. The control group was treated with conventional acupuncture, while the treatment group was treated with conventional acupuncture and acupuncture at shéntáng and nǎohù points. Both groups were treated with acupuncture once a day for six days and then rested for one day, four weeks was a course. Hamilton depression scale (HAMD), homocysteine (Hcy) level, National Institutes of Health stroke scale (NIHSS) score, activities of daily living scale (Barthel index[BI] score) were recorded and compared between two groups before and after treatment. The clinical efficacy of the two groups was compared after treatment. Results Before treatment, there were no significant differences in HAMD score, Hcy level, NIHSS score, and BI score between the two groups (P > 0.05). After treatment, HAMD score, Hcy level, and NIHSS score in the two groups were lower than those before treatment, and those in the treatment group was lower than those in the control group, the differences were statistically significant (P < 0.05). After treatment, BI scores in both groups were higher than before treatment, and that in the treatment group was higher than that in the control group, the differences were statistically significant (P < 0.05). The clinical efficacy of the treatment group was better than that of the control group, and the difference was statistically significant (P < 0.05). Conclusion Acupuncture at shéntáng and nǎohù points can improve the depressive symptoms, reduce the level of Hcy, improve the degree of neurological impairment and improve the ability of daily living in PSD patients.

Analysis of the medication rule of Professor Cheng Weiping in the treatment of Gilles de la Tourette syndrome#br#

Objective To summarize the prescription and medication rule of Professor Cheng Weiping in the treatment of Gilles de la Tourette syndrome. Methods The electronic medical record data of the outpatient Department of Professor Cheng Weiping of Heilongjiang University of Chinese Medicine from January 2018 to June 2021 were extracted, relevant medical records were screened and processed, and were input into the traditional Chinese medicine inheritance assistance platform (V3.0). Statistical methods such as frequency analysis, cluster analysis, and association rules were used to summarize the core drugs and summarize the rules of differentiation and treatment. Results A total of 94 prescriptions involving 101 Chinese medicines were included. The four nature of drugs were mainly composed of plain medicine and warm medicine. The five flavours were mainly sweet taste and bitter taste. The main channels were lung channel and heart channel. The drugs with the frequency ≥30 times included 14 herbs, such as prepared stir-frying with tortoise plastron, Gastrodiae Rhizoma, Rehmanniae Radix, and Glycurrhizae Radix Et Rhizoma Praeparata Cum Melle. The most frequently used drugs were tonify deficiency medicine. There were 19 groups of high frequency drug combinations with frequency ≥45 times; there were 33 pairs of commonly used drug combinations with confidence level ≥0.8. Four types of core drug combinations were obtained according to the results of drug cluster analysis. Conclusion Professor Cheng’s treatment of Gilles de la Tourette syndrome mainly focuses on tonifying kidney and tonifying and replenishing marrow and essence, suppressing hyperactive liver for calming endogenous wind, dispersing stagnated liver qi and softening liver.

Professor Qi Xianghua found that psychological disorder is the underlying pathogenesis of multiple system atrophy with the help of self-created the accurate identification tool of “systematic syndrome differentiation and pulse science”, among which depressed and uncomfortable state is more common. This paper discusses the pathogenesis of multiple system atrophy in depressed and uncomfortable state and analyzes the clinical features and pulse manifestation characteristics. From the psychological level, the differentiation and treatment of multiple system atrophy is preliminarily analyzed, and Xuefu Zhuyu Decoction is taken as the primary prescription for the treatment, and systematic treatment is carried out for the four classification states of spleen deficiency and phlegm excess, sinking of qi deficiency, liver fire invading lung,and yin deficiency of liver and kidney, providing a new way of thinking for clinical treatment.

Guided by the theory of collaterals disease, the etiology and pathogenesis of chloasma in Chinese medicine can be summarized as the first occurrence of the stagnation of liver qi and disharmony of qi and collaterals, root of the emptiness of zang-fu viscera and collaterals disease, and manifestation of the stagnant blockade of collaterals disease. Combined with the correlation between pathological mechanism of Western medicine and collateral disease theory, it can be summarized as vascular factor inflammatory response (collateral heat), blood circulation disorder (collateral stasis), oxidative stress response (collateral poison). Under the guidance of collateral disease theory and combined with the pathological and pharmacological mechanism of Western medicine, this paper discusses the therapeutic significance of regulating qi, tonifying deficiency, and dispelling stasis in the treatment of chloasma.

Followed up for six months, the resilience and self-management ability scores of the two groups were compared. Results Thirty-six cases in the observation group and 34 cases in the control group completed the six months follow-up. The psychological resilience scores of the two groups were higher than those before education, the total scores of toughness, self-improvement, optimism, and resilience in the observation group were higher than those in the control group (P < 0.05). The scores of self-management ability in the two groups were higher than before treatment, the total scores of ability to comply with medical drugs, diet management, disease monitoring, exercise management, and self-management of the observation group were higher than those of the control group (P < 0.05). Conclusion Multimedia health education is helpful to improve the resilience of patients with cardiovascular interventional therapy and promote the development of self-management ability.

Application value of DOHaD concept oriented dietary nursing in gestational diabetes mellitus patients#br#

Objective To explore the effect of health care and disease development origin (DOHaD) concept oriented dietary nursing in patients with gestational diabetes mellitus (GDM). Methods A total of 103 patients with GDM treated in the Second Affiliated Hospital of the Army Medical University from January 2020 to may 2021. They were divided into observation group (52 cases) and control group (51 cases) according to the random number table method. The control group was treated with routine dietary nursing, and the observation group was treated with DOHaD concept oriented dietary nursing. Both groups received nursing intervention from admission to delivery. The blood glucose control effects (fasting plasma glucose [FPG], 2 hour postprandial blood glucose [2hPG] ), self-care ability (self-care ability measurement scale [ESCA] ), the incidence of adverse pregnancy outcomes, dietary compliance and the increase of body mass index (BMI) were compared between the two groups before and after intervention. Results After the intervention, the FPG and 2hPG of the two groups were lower than those of the same group before the intervention, and the observation group was lower than that of the control group, the differences were statistically significant (P < 0.05); the ESCA scores of the two groups were higher than those of the same group before the intervention, and the observation group was higher than that of the control group, the differences were statistically significant (P < 0.05); the incidence of adverse pregnancy outcomes and the increase of BMI in the observation group were lower than those of the control group, the difference was statistically significant (P < 0.05); the diet compliance rate of the observation group was higher than that of the control group, the differences were statistically significant (P < 0.05). Conclusion DOHaD concept oriented dietary nursing can help GDM patients control weight, reduce blood sugar, improve adverse pregnancy outcomes, enhance self-care ability of patients, and improve their dietary compliance.

One case of blocking extraction of deciduous canine led to eruption of ectopic mandibular permanent canine in normal direction#br#

Ectopic mandibular canine is a rare abnormality of tooth eruption. Ectopic mandibular canine has no obvious clinical symptoms, and is often found accidentally by radiological examinations. The current diagnostic methods for ectopic canine is mainly full-mouth tomography and cone beam computerized tomography. If ectopic canine is not detected early, and prevention and blocking measures are not taken, it will lead to impacted canine. Impacted canine can lead to severe malocclusion, increase the risk of dentigerous cyst and can also root resorption of adjacent teeth, resulting in adverse effects on the masticatory function, aesthetics, and psychology of patients. This article reports one case of blocking extraction of deciduous canine in the early stage of ectopic mandibular permanent canine and the removal gap was managed and achieved good results. Combined with the literature review, it was found that early prevention or blocking extraction of deciduous canine is the most ideal treatment method. Early intervention can improve the degree of ectopic canine, reduce the risk of impacted canine, and contribute to the stability of the oral and maxillofacial system and the health of adjacent teeth.

Funding status and characteristics analysis of the National Natural Science Foundation of traditional Chinese medicine rheumatism and immunity from 2010 to 2019#br#

Objective To understand the funding status of the National Natural Science Foundation of traditional Chinese medicine rheumatism and immunity from 2010 to 2019, and to conduct characteristic analysis. Methods From January 2019 to August 2020, the National Natural Science Fundation of China website “shared service network” collected in January 2010 to December 2019, nearly 10 years of general project of National Natural Science foundation, youth science fund project, the district science fund projects, and key projects in the field of rheumatic immunity of traditional Chinese medicine (internal Chinese medicine and combining Chinese and Western medicine) were analyzed. Results A total of 389 projects were funded from 2010 to 2019. The institutions with the highest number of funded projects were universities of Chinese medicine in Zhejiang, Shanghai, Jiangsu, Fujian, Guangdong, and other places. For more than two consecutive funding projects, Zhejiang Chinese medicine university (9 people, 20 items), Fujian University of traditional Chinese medicine (5 people, 14 items), Guang’anmen Hospital of China Academy of Chinese Medical Sciences (6 people, 13 items), Beijing University of Chinese Medicine (5 people, 11 items), Southern Medical University (4 people, 11 items) were among the top five. The top six diseases in terms of funding amount were rheumatoid arthritis, osteoarthritis, systemic lupus erythematosus, gout and hyperuricemia, ankylosing spondylitis, and sjogren’s syndrome. Conclusion The National natural Science Foundation of China has continuously increased in the number and amount of funding in the field of traditional Chinese medicine rheumatism, because some universities and hospitals of traditional Chinese medicine have a deep foundation and focus on discipline development, and have many funded projects. Diseases of funded projects are unbalanced.

Health education service is an important measure to achieve the equalization of basic public health services and the strategic goal of Healthy China, it is mainly carried out in community, but the implementation effect is not satisfactory. The service model of community health education should be explored and reformed. The grid service model of community health education is a new model under the background of grid management in recent years. However, the integration and development of health education and grid management is restricted by many factors. The pandemic has catalyzed the transformation of grid management and the development of health education, bringing an opportunity for the integration of them. In order to consolidate the achievements during resistance to pandemic, and play the synergistic role of community grid management in health education, we should seize the development opportunity to carry out practical pilot studies as soon as possible, strengthen government support, establish a long-term cooperation mechanism, accelerate the construction of social governance system, play the advantages of the Internet, explore the feasible path of health education grid service model, and promote the sustainable development and comprehensive advancement of the grid service model of community health education.
