JIANG Chenyang LI Shuo CHAO Xiaoying ZHONG Guoqiang
Objective To provide a new bioinformatics basis for the diagnosis and treatment of the disease, we analyzed the key genes and potential drugs of acute myocardial infarction (AMI) complicated with chronic obstructive pulmonary disease (COPD) based on bioinformatics. Methods Three data sets containing AMI and COPD microarray information were downloaded from the GEO database and differentially expressed genes (DEG) were screened. GO function annotation, KEGG pathway enrichment analysis, and Metascape enrichment analysis were performed on the differentially expressed genes shared by the two diseases. Then a protein protein interaction (PPI) network was constructed to identify key genes involved in the occurrence and development of diseases. Finally, the drug-drug gene interaction database was used to find potential therapeutic drugs. Results After integrating the three data sets of AMI and COPD, 61 DEGs were obtained. The results of GO function annotation showed that they were mainly involved in inflammation, carbohydrate and glucocorticoid receptor binding. Mostly located in the outer membrane of organelles and the inner cavity of particles. KEGG and Metascape enrich the pathways such as immune, inflammation, and apoptosis. Eight key genes identified by the PPI network, a total of 74 potentially related therapeutic drugs were screened. Tuberculin, tomalizumab, resveratrol caproic acid, tasquinide, and pannimod were highly associated with diseases. Conclusion The identified genes and pathways may be potential therapeutic targets for AMI combined with COPD, and provide a new basis for the molecular mechanism of the disease.

Research situation of the syndrome differentiation of micro-indexes of chronic heart failure
ZHONG Senjie LI Jing LI Lin HU Siyuan YANG Meng ZHANG Qian XIONG Xiajun HU Zhixi
The typical external symptoms and signs of chronic heart failure appear relatively late, while the traditional four-diagnostic method failed to go deep into the micro level, which seriously restricts the development of syndrome differentiation. Micro-indexes obtained by modern detecting techniques can enrich the visual observation content of external images, and make up for the limitations of the traditional four-diagnostic method, thereby gradually becoming beneficial complements to the syndrome differentiation of chronic heart failure. This paper summarizes the research situation of micro-indexes such as cardiac function classification and related indicators, natriuretic peptides secreted by the heart, biological markers and other micro indexes in the syndrome differentiation of chronic heart failure, analyzes the correlation between different micro-indexes and the evolution of chronic heart failure syndromes, discusses the diagnostic value and limitations of each micro-indexes, and discussse the limitations of single index and the lack of specificity of some indexes at present, and puts forward some development ideas, such as introducing the concept of weight quantification, combined application of multiple indexes and the aid of system biology, in order to provide new ideas for the objectification of chronic heart failure syndrome differentiation.

TANG Yingchun DONG Xiongjun TANG Lijuan WANG Zhengxiang ZHANG Jing▲
Objective To explore the survival of diabetic nephropathy hemodialysis patients and influencing factors. Methods A total of 95 patients confirmed diabetic nephropathy and underwent hemodialysis admitted to the Second People’s Hospital of Wuhu, Anhui Province from February 2014 to February 2018 were followed up. Follow-up ended in February 2021, with death as the study’s termination event. The general data, blood biochemical, and cardiovascular disease indicators were collected. Cox regression was used to analyze the risk factors of death in diabetic nephropathy hemodialysis patients. Results The follow-up time ranged from 16 to 54 months, with a median follow-up time of 30.5 months. At the end of follow-up, 52 of all cases survived and 43 cases died. Estimated glomerular filtration rate and total cholesterol levels of the dead patients were lower than those of the surviving patient and the proportion of C-reactive protein, low density lipoprotein, blood phosphorus, proportion of patients with coronary heart disease, and myocardial ischemia were higher than those of the surviving patients, with statistical significance (P < 0.05). Multivariate analysis showed that high blood phosphorus, complicated with coronary heart disease and myocardial ischemia were independent risk factors for death in diabetic nephropathy patients on hemodialysis (HR > 1, P < 0.05). Conclusion During hemodialysis treatment for diabetic nephropathy patients, attention should be paid to the prevention of coronary heart disease and myocardial ischemia, and the reduction of blood phosphorus level.

FU Jiarong ZHU Yuchen YU Jie LIU Wei TAO Tao
Objective To investigate the relationship between triglyceride-glucose index (TyG) and the risk of non-alcoholic fatty liver disease (NAFLD) in patients with polycystic ovary syndrome (PCOS). Methods A total of 350 PCOS patients admitted to the Department of Endocrinology and Metabolism, Renji Hospital, Shanghai Jiao Tong University School of Medicine from January 2017 to December 2020 were selected. According to whether they had NAFLD or not, they were divided into NAFLD group (201 cases) and non-NAFLD group (149 cases). TyG, lipid accumulation index (LAP) and visceral adiposity index (VAI) were calculated. Spearman correlation coefficient was used to analyze the correlation between TyG and liver function, glucolipid metabolism and reproductive indexes. Logistic univariate and multivariate analysis was used to analyze the correlation between TyG, LAP, VAI, and NAFLD in PCOS patients. Results There were statistically significant differences in clinical indicators, TyG, LAP, and VAI between the two groups (P < 0.05). Correlation analysis showed that TyG was positively correlated with glutamic-pyruvic transaminase, glutamic-oxalacetic transaminase, γ-glutamyl transpeptidase, total cholesterol, low density lipoprotein cholesterol, area under insulin curve and free androgen index (rs > 0, P < 0.05). And it was negatively correlated with high density lipoprotein cholesterol, sex hormone-binding globulin, and anti-Müllerian hormone (rs < 0, P < 0.05). Univariate logistic regression analysis showed that age, body mass index, homeostasis model assessment-IR, free androgen index, glutamic-pyruvic transaminase, TyG, LAP, and VAI were all risk factors for NAFLD in PCOS patients (OR > 1, P < 0.05). In order to compare the relationship between TyG, LAP, VAI, and NAFLD in PCOS patients and avoid the interaction between the indexes, they were stratified by quartile, and the subjects were included in four subgroups of every index, respectively. Multivariate logistic regression was performed to correct confounding factors. The results showed that TyG > 7.95, LAP > 15.85, and VAI > 1.91 were independent risk factors for NAFLD in PCOS patients (OR > 1, P < 0.05). Conclusion TyG index has certain value for the auxiliary diagnosis of NAFLD in PCOS patients.

Clinical experience of Professor Mao Dexi in treating palpitation based on syndrome differentiation
PAN Haitao1 LU Yongping2
Professor Mao Dexi, the first national famous Chinese medicine doctor, believes that the main pathogenesis of palpitation is “deficiency complicated with excess”. The deficiency is mostly caused by the insufficiency of pectoral qi, and the excess is mostly caused by blocked venation. The lack of pectoral qi and the blocked of the venation often interact with each other, which causes palpitation. Professor Mao Dexi pays attention to the combination of constitution and syndrome in the aspect of syndrome differentiation, pays attention to invigoration and interflow in the aspect of prescription-medicine application. In clinic, palpitation is often divided into six common syndrome types, including insufficiency of qi and blood, devitalization of heart yang, timidity due to deficiency of heart qi, phlegm-fire disturbing heart, blood stasis blocking heart vessel, qi deficiency and blood stasis. Based on the study of the medical books of ancient doctors, and combined with his own clinical experience, Professor Mao Dexi makes “Xinji Bawei Prescription” by himself in the treatment of palpitation, which has the effects of benefiting qi and replenishing blood, promoting blood circulation for removing blood stasis, calming mind and palpitation, and the effect is remarkable in clinical treatment.

Correlation between cerebral glucose metabolism and cognitive function in Alzheimer’s disease patients based on PET/CT
WANG Yanyan1 LI Xiaoxia2 XIA Huan3 LUO Qin4
Objective To explore the correlation between cerebral glucose metabolism and cognitive function in Alzheimer’s disease patients based on positron emission tomography / computed tomography (PET/CT). Methods Clinical data of 31 patients with primary AD in Gansu Provincial People’s Hospital (hereinafter referred to as “our hospital”) from September 2016 to October 2019 were analyzed. According to the mini-mental state examination (MMSE), the patients were divided into mild to moderate AD group (22 cases, 12≤MMSE< 24 points) and severe AD group (9 cases, MMSE< 12 points). The control group was 57 healthy people who underwent physical examination in our hospital during the same period. In PET/CT examination, the imaging agent was fluorodeoxy glucose (18F-FDG), the image fusion and reconstruction were processed by PHILIPS Syntegra software, and SUVavg (Br/Bl) value was obtained after the correction of interest area. At the same time, the correlation between brain glucose metabolism and cognitive function in AD patients was analyzed. Results There was a positive correlation between SUVavg (Br/Bl) value in bilateral temporal lobe and recall power score in MMSE score in AD patients (r = 0.516, P < 0.05). SUVavg (Br/Bl) value of right parietal lobe in mild to moderate AD group was lower than that in control group (P < 0.05). Conclusion The progressive decrease of 18F-FDG glucose metabolism in temporal lobe of AD patients is consistent with the decline of the ability to memorize score in MMSE, and PET/CT can earlier reflect the functional damage of right parietal lobe in mild to moderate AD patients.

Molecular epidemiology and drug resistance mechanism analysis of carbapenem-resistant Klebsiella pneumoniae isolated from patients and object surface in the intensive care unit
CHEN Tao TANG Jiao DU Jingjing LIU Xiaohua CHEN Jing
Objective To understand the drug resistance mechanism and molecular epidemiological characteristics of carbapenem-resistant Klebsiella pneumoniae (CRKP) isolated from patients and object surface in the intensive care unit (ICU), and to adopt reasonable strategies to block its transmission in hospital based on the actual situation of this ward, so as to provide an important basis for clinical rational drug use and nosocomial infection control. Methods A total of 29 strains of CRKP isolated from ICU patients and object surface in Xindu District People’s Hospital of Chengdu (hereinafter referred to as “our Hospital”) from July 2019 to June 2020 were collected. The antimicrobial resistance of common clinical antibiotics was detected by MicroScan WalkAway40 bacterial susceptibility analyzer and paper diffusion method. Five common carbapenem-resistant genes were detected by polymerase chain reaction (PCR) amplification and DNA sequence analysis. The gene correlation among the strains was detected by multilocus sequence typing (MLST) and pulsed field gel electrophoresis (PFGE). Results A total of 29 strains of CRKP were sensitive to Tigecycline and Polymyxin, and had high sensitivity to Amikacin and Gentamicin, and high resistance to other common antibacterial drugs. PCR and DNA sequencing showed that among 29 strains of CRKP, only one strain was KPC-2 type. The other 28 strains were NDM-1 type. No other resistance genotypes were detected. MLST and PFGE detection results showed that 29 strains of CRKP were classified into six types, with ST12 type as the main type. A total of 22 strains of ST12 type CRKP, 11 strains of ST12/A type and six strains of ST12/B type were detected. The object surface strain was ST12/A type. Conclusion In ICU ward of our hospital, NDM-1 carbapenemase ST12/A type is the main type of CRKP, and the isolated strains on the object surface are consistent with the epidemic strains in this area, with high homology. Relevant departments should strengthen the intervention of nosocomial transmission of CRKP, clarify the transmission route, and take a series of prevention and control measures to prevent the spread of drug-resistant strains in the hospital.
