XU Yuxuan1 YAN Zhanpeng2 XU Tingting2 WANG Pei1 ZHENG Xue1 ZHU Fangshi1,2
Objective To observe the effect of Jianpi Yiqi Formula on gastric tissue of rats with chronic atrophic gastritis (CAG) with syndrome of spleen deficiency. Methods Fifty-five healthy SD rats were selected to make the model of rat with CAG with syndrome of spleen deficiency by MNNG + Ranitidine method. After 24 weeks of intervention, five of them were selected to observe the morphology of their gastric mucosa to confirm the success of modeling. The last rats were divided into Jianpi Yiqi Formula low (13.1 g/ [kg·d]), medium (26.2 g/ [kg·d]) and high dose group (52.4 g/ [kg·d]), and model group (normal saline), positive drug group (folic acid 2.7 mg/ [kg·d] + Teprenone 13.5 mg/ [kg·d]) by random number table method, with ten rats in each group, another ten healthy SD rats were selected as blank group (normal saline). The status and body weight of the six groups were recorded by gavage for eight weeks. The pathological changes of the gastric tissues were observed by HE method. The changes of serum gastrin and somatostatin (SST) levels were determined by enzyme linked immunosorbent assay, and the protein expressions of Ki67, Notch1, Notch2 and Hes1 were determined by immunohistochemical stain. Results Blank group fur was white and shiny, active. The fur of the model group was yellow and easy to fall off, and the activity was reduced. The positive drug group and the low-dose, medium-dose and high-dose groups of invigorating spleen and replenishing qi formula had bright fur and high activity. The body weight of the model group was lower than that of the blank group, and the body weight of the Jianpi Yiqi Formula medium-dose and high-dose groups were higher than that of the model group, with statistical significance (P < 0.05 or P < 0.01). The results of HE staining showed that the gastric mucosa epithelial layer was clear and the glands were normal in the blank group. Gastric mucosal epithelium in model group was thinned, glands were reduced, arranged loosely and disorderly, and goblet cells could be seen. In the other four groups, the gastric mucosal epithelium was thickened, the glands were increased, the arrangement tended to be orderly, and the infiltration of inflammatory cells was decreased. Serum gastrin and SST levels in model group were lower than those in blank group, while the levels of serum gastrin in Jianpi Yiqi Formula high-dose group was higher than that in model group, and the levels of SST in Jianpi Yiqi Formula low-dose, medium-dose and high-dose groups were higher than that in model group, with statistical significance (P < 0.05 or P < 0.01). The protein expressions of Ki67, Notch1, Notch2 and Hes1 in model group were lower than those in blank group, the protein expression level of Ki67 in Jianpi Yiqi Formula low-dose group was higher than that in the model group, the protein expression levels of Notch1 and Notch2 in Jianpi Yiqi Formula low-dose, medium-dose and high-dose groups were higher than that in the model group, and the protein expression level of Hes1 in Jianpi Yiqi Formula high-dose group was higher than that in the model group, with statistical significance (P < 0.05 or P < 0.01). Conclusion Jianpi Yiqi Formula can improve the pathological state of gastric mucosa atrophy in CAG rats with syndrome of spleen deficiency, and the mechanism is related to the up-regulation of the expression of proteins related to Notch and other signaling pathways.

WANG Xun1 WANG Dongping2 CAI Yang1
Objective To investigate the expression of T cell immunoglobulin domain and mucin domain protein-3 (Tim-3), galectin-9 (Gal-9) and interferon-γ (IFN-γ) in oral lichen planusand (OLP) tissues and the role of Tim-3/ Gal-9 pathway in the formation of OLP and its significance. Methods From October 2016 to October 2019, 41 cases of OLP patients treated in the Department of Periodontal Mucosa, Stomatological Hospital of Guizhou Medical University (“our hospital” for short) were selected as the OLP group, and 10 normal oral mucosal people treated in our hospital during the same period were selected as the normal group. The expression of Tim-3, Gal-9, and IFN-γ in the two groups and their correlation were detected by immunohistochemical SABC method, at the same time, the correlation between the pathological characteristics of OLP patients and Tim-3, Gal-9 and IFN-γ were analyzed. Results The positive expression rates of Tim-3 and IFN-γ in the OLP group were higher than those in the normal group, and the positive expression rates of Gal-9 was lower than that in the normal group, with statistical significance (P < 0.05). In OLP tissue, the expression of Tim-3 was positively correlated with the epithelium and lamina propria (r > 0, P < 0.05); the expression of Tim-3 and Gal-9 was negatively correlated in the epidermis (r < 0, P < 0.05); and the expression of Tim-3 and IFN-γ was positively correlated in lamina propria (r > 0, P < 0.05). The score of lymphocyte infiltration was positively correlated with the positive expression of Tim-3 in the epithelium and lamina propria (r > 0, P < 0.05), and negatively correlated with Gal-9 (r < 0, P < 0.05); the score of liquefaction infiltration of basal cells was positively correlated with the positive expression of Tim-3 and IFN-γ in the epidermis (r > 0, P < 0.05). Conclusion The high expression of Tim-3 and low expression of Gal-9 in the lesions of OLP patients may be due to the abnormal Tim-3/Gal-9 pathway, which enhances T helper cell 1 effector cells and promotes the secretion of IFN-γ, and is involved in the process of pathological injury in the lesion area of OLP.

38 Research progress on immune regulation mechanism of external therapy of traditional Chinese medicine on inflammatory bowel disease
JIAO Yi1 ZHANG Yingqi1 LU Mengqian1 YU Tianyuan1 LIU Zhifeng1 LIU Di2
Inflammatory bowel disease (IBD) is an autoimmune disease with lesions located in the mucosa of colon and rectum. At present, IBD is believed to be mostly related to diet, mood, infection and immunity, among which immunity is the most studied. Four traditional Chinese medicine external treatment methods are mostly selected clinically, including acupuncture, moxa-wool moxibustion, massage and traditional Chinese medicine enema therapy. In this paper, from the point of inflammation and immunity of IBD, the immune regulation mechanism of external treatment of Chinese medicine on IBD is discussed. The external treatment of traditional Chinese medicine can regulate the inflammatory immune response of the body through multiple targets and channels, so as to achieve the state that the vital qi is stored inside and the pathogen cannot be dried. The four kinds of external therapy intervention methods involved in this paper are different, and the mechanisms involved are also different. Acupuncture provides mechanical stimulation, moxa-wool moxibustion provides warm stimulation, and massage provides both mechanical stimulation and warm stimulation, and traditional Chinese medicine enema therapy provides drug stimulation. Acupuncture and moxa-wool moxibustion are both involved in balancing immune microenvironment and anti-inflammatory mechanism. Acupuncture is also involved in anti-oxidative stress response, anti-endoplasmic reticulum stress response and the alleviation of visceral sensitivity. Massage mainly provides sufficient morphological basis, and traditional Chinese medicine enema therapy regulates immune disorders through direct contact with the lesion site.

114 Reflection on improving the curative effect of pain by blending Western medicine into Chinese medicine
NIU Qun1 WANG Huan2 WU Ruixin3 ZHANG Shaoyou4 DONG Changsheng1,5
As a complex subjective experience, pain can occur in many parts of the patient’s body and is often the main reason for them to seek medical attention. Traditional Chinese medicine has a history of thousands of years in the treatment of pain, as early as in Huangdi Neijing has systematically elaborated the cause of pain, pathogenesis, syndrome, prognosis and so on. It has formed its unique treatment system under the continuous supplement and improvement of many doctors in the later generations. But up to now, there are many disadvantages in pain treatment of traditional Chinese medicine, such as the great influence of subjective factors, difficulty in standardization and high difficulty in clinical promotion, which are not conducive to the development of pain theory of traditional Chinese medicine. Therefore, this paper applies traditional Chinese medicine thinking to integrate modern medical theories and methods into traditional Chinese medicine pain treatment and research, and probes into the molecular biological connotation of traditional Chinese medicine pathogenesis based on pain signal transmission, which may play a positive role in the study of traditional Chinese medicine pain, in order to improve the treatment effect of pain.

118 Treatment of stroke from the replenishing liver and kidney based on Huang Yuanyu’s theory of “one qi circulation”
LIU Zehua1 YAN Yongmei1 LI Tao1 WANG Dou2
Most physicians in the past dynasties believe that the cause of stroke is closely related to insufficient qi and blood and deficiency of liver and kidney. Huang Yuanyu’s theory of “one qi circulation” has been innovative on the basis of inheriting the classic theory. He believes that the deficiency of middle qi runs through the whole course of stroke. Based on the holistic view of “one qi circulation and soil drives four emblems”, he concludes that the pathogenesis of stroke is deficiency of primordial qi, deficiency of middle qi, cold water and soil dampness, and stagnation of wood that lead to liver wind symptoms, liver wood inability to grow and stretch, leading to stroke. The author uses the theory of “one qi circulation” to analyze the internal mechanism of stroke “yin-yang disharmony, qi and blood reverse disorder”, and concludes that the primary cause of stroke is deficiency of qi and soil dampness, and the fundamental cure is to replenish liver and kidney. The treatment should focus on regulating the rise of the left path of the kidney, spleen and liver in the “one qi circulation”. To replenish the liver and kidney focuses on “making cold water warm” instead of “making depleted water abundant”. By warming the kidney and water to warm yuanyang, drying the spleen and soil to consolidate the middleqi, benefiting the qi of liver to make the pivot flexible, to achieve a good prognosis of mild soil and water, flourishing liver and wood, and quiet wood and wind. The theory of “one qi circulation” has important clinical guiding significance in differentiating and treating stroke, which is worthy of further study.

134 Professor Zheng Shaozhou’s experience in treating multiple sclerosis from “endotoxin”
GUO Yanyan1 GONG Hongtao2 ZHAO Duo2
This paper sorts out Professor Zheng Shaozhou’s experience in treating multiple sclerosis from the theory of endotoxin, and the primary responsibility for the pathogenesis of multiple sclerosis lies in the kidney. It is closely related to the liver and spleen. Deficiency of viscera leads to endotoxin, endogenous turbid toxin is a key factor in the pathogenesis and recurrence of multiple sclerosis. The collaterals are the transmission pathways of toxins, so the treatment of detoxification and collaterals runs through the treatment of the disease, differentiation and treatment of the internal organs, detoxify, cutting off the conversion chain of endotoxin, and treatment by stages. In the acute attack stage, it is mainly evil and excess, the main focus is to detoxify and dredge collaterals, as well as tonifying the kidney; in the chronic progression stage, deficiency is the mainstay or a mixture of deficiency and excess, aiming to balance yin and yang, replenish viscera, and dispel evil; in remission stage, deficiency of healthy qi, loss of blood yin and yang, internal poisonous evil, mixed deficiency and excess, yin and yang interlaced, treatment must focus on strengthening the body, taking into account the elimination of pathogens, adjusting the balance of yin and yang in the viscera, and fundamentally removing the internal factors of the occurrence and development of the disease. The dependent nature of toxins, treatment rules, and prescriptions have their own emphasis. The prescription is selected according to the evidence, and the case is analyzed to grasp the main line. The effect is quite good.

FENG Sili1 CHEN Yaqin1 SUN Hairong1 GUO Chunling2 LIU Mei2▲
Objective To observe the self-care ability after discharge of patients with acute myocardial infarction (AMI) after percutaneous coronary intervention (PCI), and to analyze the influencing factors of self-care ability after discharge of patients. Methods A total of 200 patients who met the treatment criteria for PCI and received PCI were selected from Xuzhou Cancer Hospital, Jiangsu Province from June 2018 to June 2020. After discharge, the patients were followed up for three months. At the end of follow-up, the patients were divided into low level group (≤57 points, 81 cases) and medium  high level group (≥58 points, 119 cases) according to their exercise of self-care agency scale scores. A general data questionnaire was used to record the baseline data of patients and were compared. The general self-efficacy scale (GSES) was used to evaluate their self-efficacy. Multivariate analysis was used to analyze the factors affecting the self-care ability of patients with AMI treated by PCI after discharge. Results Among the 200 patients, 81 cases (40.50%) had low self-care ability, 65 cases (32.50%) had moderate self-care ability, and 54 cases (27.00%) had high self-care ability. The proportion of negative emotions and one to two kinds of medication in the low level group were higher than those in the middle and high level group, and the difference was statistically significant (P < 0.05). There were statistically significant differences in educational level and GSES scores between the two groups (P < 0.05). Multivariate analysis showed that low education level, negative emotion and one or two kinds of medication were independent risk factors for low self-care ability of AMI patients treated with PCI (all OR > 1, P < 0.05), and high GSES score was the protective factor (OR < 1, P < 0.05). Conclusion The level of self-care ability of AMI patients with the treatment of PCI is low after discharge, which may be affected by the factors such as low educational level, negative emotion, less types of medication and low self-efficacy, clinical should focus on the above factors to give targeted intervention measures.

185 A case of multiple system atrophy with inappropriate antidiuretic hormone secretion syndrome combined with central fever treated with integrated traditional Chinese and Western medicine
ZHANG Chen1 HAO Conglin2 SHANG Juju1 ZHOU Xusheng1 ZHAI Junpeng1 LI Jing1
Multiple system atrophy (MSA) is a rare and relatively rapid neurodegenerative disease involving the three major systems of autonomic nervous system, extrapyramidal system and cerebellum. Syndrome of inappropriate antidiuretic hormone secretion (SIADHS) is a clinical syndrome in which excessive secretion of antidiuretic hormone (ADH) leads to body water retention, dilutional hyponatremia, and elevated urine sodium and urine osmotic pressure. Case reports of MSA combined with SIADHS are rare. Combined with central fever, the prognosis is worse. Western medicine treatment is not effective. Clinically, it has been shown that combined traditional Chinese and Western medicine is more effective than Western medicine alone. This article reports the diagnosis and treatment process of a case of MSA with inappropriate antidiuretic hormone secretion syndrome combined with central fever treated with mtegrated traditional Chinese and Western medicine, showing that Qinghao Biejia Decoction has a good effect on the treatment of this disease. At the same time, a literature review is carried out in order to strengthen the understanding of the disease and to explore the advantages of traditional Chinese medicine in the treatment of the disease.

193 Enlightenment of “Invisible Narrative” in traditional Chinese medicine to narrative medicine
WANG Jinglin1 FU Xinjun1 LI Yajun1,21.College of Humanities and management, Shaanxi University of Chinese Medicine, Shaanxi Province, Xianyang 712046, China; 2.Research Center of Traditional Chi
In the face of the increasingly prominent ideological value-related issues in the global public health field, narrative medicine has emerged as the times require. Exploring the characteristics of traditional Chinese medicine in that the humanistic spirit runs through the diagnosis and treatment of diseases, so the “narrative” nature of traditional Chinese medicine is inherent. In diagnosis and treatment, traditional Chinese medicine practitioners record parallel medical records with the patient as the protagonist through the “four diagnoses combined with reference” and “seeing the micro-knowledge”, and combine pathogenesis, syndrome differentiation, and prescription elements to form an “invisible narrative” logic. Finally, both doctors and patients can truly understand and trust on the spiritual level. This fully embodies the three elements of narrative medicine: attention, reproduction and belonging. Based on this, the concept of “invisible narrative” in Chinese medicine is explored under the current medical situation in China, in order to provide a reference for narrative medicine to achieve the ultimate goal of doctor-patient empathy.
