4 Exploring mechanism of Astragali Radix in the treatment of bronchial asthma on network pharmacology

Objective To explore the possible mechanism of Astragali Radix in the treatment of bronchial asthma by network pharmacology. Methods The effective chemical ingredients and corresponding targets of Astragali Radix were obtained based on traditional Chinese medicine systems pharmacology database and analysis platform. The targets related to bronchial asthma were obtained through Genecards database, and the potential targets of Astragali Radix acting on bronchial asthma were obtained by mapping Astragali Radix targets and disease targets. Astragali Radix-compound-target-disease network was constructed by Cytoscape software. Proteinprotein interaction (PPI) data were obtained from the STRING online database and imported into Cytoscape to construct PPI network. The gene ontology (GO) function enrichment analysis and Kyoto encyclopedia of genes and genomes (KEGG) pathway enrichment analysis of the targets was carried out using the Bioconductor bioinformatics software package. Results Nineteen active compounds of Astragali Radix were screened and obtained. Ninety-seven predicted targets of active compounds of Astragali Radix were obtained. A total of 5973 targets related to bronchial asthma were found in Genecards database. A total of 81 potential targets of Astragali Radix on bronchial asthma were obtained. The results of GO and KEGG enrichment analysis showed that DNA and RNA transcription regulation, activity and receptor binding of cytokines and transcription factors, anti-oxidant stress, hormone regulation, protein binding, enzyme activity and other biological processes were mainly involved by adjusting AGE-RAGE signaling pathway, TNF signaling pathway, PI3K-Akt signaling pathway, p53 signaling pathway and other pathways to exert its treatment of bronchial asthma. Conclusion Astragali Radix and its active ingredients exert anti-inflammatory, anti-oxidation, immune regulation and other effects through various targets and multiple pathways, and play a therapeutic role in inflammation, airway hyperresponsiveness and airway remodeling caused by bronchial asthma.

9 Discussion on the prescription rules of medicinal diet for qi deficiency constitution based on data mining

Objective To analyze the prescription rules of medicated diet for qi deficiency constitution, so as to provide theoretical basis for regulating qi deficiency constitution with food therapy in the future. Methods The prescriptions of Chinese herbal food dictionary and Chinese herbal food science for qi deficiency constitution were screened. The types and frequency of medicated food and raw materials were counted. Cluster analysis and association rule analysis were carried out by software. Results Three hundred and forty-six prescriptions were selected, involved 216 kinds of raw materials, included 111 kinds of food materials and 105 kinds of traditional Chinese medicine. The top three ingredients used were white sugar, hen and glutinous rice, while traditional Chinese medicines were Astragali Radix, Codonopsis Radix and Jujubae Fructus. Cluster analysis showed that the raw materials could be divided into 10 categories, and 17 association rules were found. Conclusion Qi deficiency constitution medicated diet is mainly composed of herbs for replenishing qi and invigorating spleen, supplemented by herbs for diuresis and dampness, tonifying blood and activating blood, and food materials with the effect of replenishing qi.

13 Protective effect of triptolide on retinal ganglion cells in rats with acute ocular hypertension

Objective To observe the protective effect of triptolide on retinal ganglion cells (RGCs) in rats with acute ocular hypertension. Methods Sixty Wister rats were selected, male and female in half. The rats were divided into control group, model group and Triptolide group by random number table method, with 20 rats in each group. Model group and Triptolide group were modeled by anterior chamber perfusion and pressure method. After successful modeling, the Triptolide group was intraperitoneally injected with 10 μg/kg of Triptolide daily, while the control group and model group were intraperitoneally injected with the same amount of normal saline daily, for a total of seven days. The pathological changes of retinal layers were observed by HE staining. The apoptosis of RGCs was observed by TUNEL staining and the survival was observed by immunofluorescence method. The expression of caspase-3 in the retinas of the three groups was detected by immunohistochemistry. Results In the control group, the retinal layers were arranged neatly and the nuclear boundaries were clear. The retinal layers of the model group were irregular and atrophied, while the retinal layers of the triptolide group were clear and orderly. The thickness of retinal nerve fiber layer (RNFL), the number of apoptotic RGCs and the number of caspase-3 positive cells in model group were higher than those in control group, and the number of survival RGCs was lower than that in control group, with statistical significance (all P < 0.05). The thickness of RNFL, number of apoptotic RGCs and number of caspase-3 positive cells in triptolide group were lower than those in model group, and the number of survival RGCs was higher than that in model group, with statistical significance (all P < 0.05). Conclusion Triptolide has a significant protective effect on RGCs in rats with acute ocular hypertension.

17 Overactivation of nucleotide-binding oligomerization domain-like receptor protein 3 is involved in lung injury induced by acute respiratory distress syndrome and injurious mechanical ventilation

Objective To investigate whether nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3) inflammasome involved in acute respiratory distress syndrome (ARDS) and lung injury induced by invasive mechanical ventilation. Methods Ten SD rats were divided into normal saline group (NS, n = 5) and oleic acid group (OA, n = 5) according to random number table method. NS group and OA group were given 0.1 mL /Kg of NS and OA via jugular vein, respectively. Two hours later, the ARDS model was determined to be successful by oxygenation index, lung injury score and lung wet-to-dry weight ratio (W/D). Another 40 male SD rats were divided into control group (n = 10) and OA group (n = 30). Two hours after anesthesia, according to different ventilation mode, OA group was divided into ARDS group, neap tide volume group (VT = 6 mL/kg) and injury ventilation group (VT = 20 mL/kg), with 10 rats in each group. The control group and ARDS group maintained spontaneous breathing, and the latter two groups were given mechanical ventilation. Carotid blood was collected at 2, 3, 4, 5 h and 6 h after anesthesia for blood gas analysis, W/D and lung injury score 6 h later were measured. RT-PCR and Western blot were used to detect the mRNA and protein expressions of NLRP3, caspase-1, interleukin (IL)-1β and IL-18 in lung tissues, respectively. Results W/D and lung injury scores in OA group were higher than those in NS group, and oxygenation index was lower than that in NS group (P < 0.01). The lung injury score and W/D in ARDS group were lower than those in injury-induced ventilation group, and higher than those in neap tide volume group and control group (all P < 0.01). The pH and oxygenation index of ARDS group were lower than those of neap tide volume group, and higher than those of injury ventilation group (all P < 0.05). Arterial partial pressure of carbon dioxide (PaCO2) in ARDS group was higher than that in injury ventilation group and lower than that in neap tidal volume group (all P < 0.05). The protein and mRNA expressions of NLRP3, caspase-1, IL-1β and IL-18 in ARDS group were lower than those in injury ventilation group and higher than those in control group (all P < 0.01). The mRNA expression of NLRP3, caspase-1, IL-1β and IL-18 in ARDS group and neap tidal volume group had no statistical significance (all P > 0.05). Conclusion Overactivation of NLRP3 inflammasome is involved in ARDS and lung injury induced by invasive mechanical ventilation, intrusive mechanical ventilation further activates NLRP3 inflammasomes and aggravates lung injury, neap tidal ventilation prevents further activation of NLRP3 inflammasomes.

22 The role and mechanism of hypoxia in Erastin-induced ferroptosis of pancreatic ductal adenocarcinoma cells

Objective To study the role and mechanism of hypoxia in Erastin-induced iron death of pancreatic ductal adeno carcinoma (PDAC) cells. Methods PDAC cells were divided into four groups, normoxia DMSO group, normoxia Erastin group (5.0 μmol/L Erastin solution), hypoxia DMSO group, and hypoxia Erastin group (5.0 μmol/L Erastin solution). After 72 hours of incubation, the cell activity was detected by CCK-8 method; full-wavelength microplate reader detects the content of reduced glutathione (GSH) and malondialdehyde (MDA); RT-qPCR detected the expression level of the deacetylase Sirtuin3 (SIRT3) mRNA; animal subcutaneous tumor formation experiments were used to detect the effects of hypoxia on iron death in vivo. Results The PANC1 cell activity, the relative content of reduced GSH, and the relative expression of SIRT3 mRNA in the hypoxic Erastin group were higher than those of the normoxic Erastin group, and the differences were statistically significant (P < 0.05). The relative content of MDA in the hypoxic Erastin group was lower than that of the normoxic Erastin group (P < 0.05). The tumor volume of the normoxic Erastin group was smaller than that of the normoxic DMSO group, and the difference was statistically significant (P < 0.05). There was no significant difference in tumor volume between the hypoxic Erastin group and the hypoxic DMSO group (P > 0.05). Conclusion Hypoxia inhibits the iron death of PDAC induced by Erastin, and the increased expression of SIRT3 is one of its mechanisms.

26 Study on content determination of total flavonoids, polysaccharides, inorganic salts and five active components in Huagan Decoction of classical prescriptions

Objective To establish a method for the determination of total flavonoids, polysaccharides and inorganic salts in 16 batches of Huagan Decoction, and to establish a high performance liquid chromatography (HPLC) method for the simultaneous determination of five active components in Huagan Decoction, which provides a comprehensive basis for the quality control of the classic recipe Huagan Decoction. Methods The contents of total flavonoids and polysaccharides in Huagan Decoction were determined by ultraviolet-visible spectrophotometry, and the methodology was investigated. The content of inorganic salts was determined by ash determination method. The determination was performed on Kromasil 100-5-C18(250 mm×4.6 mm, 5 μm) column with mobile phase consisted of acetonitrile-0.05% phosphoric acid solution (gradient elution) at the flow rate of 0.9 mL/min. The column temperature was 30℃. The detection wavelength was 230 nm, and the methodology was investigated. Results The average content of total flavonoids, polysaccharides and inorganic salts in the 16 batches of Huagan Decoction were about 2.99%、13.92%, 8.55%; the linear ranges of hesperidin, D-glucose anhydrous were 0.0051-0.0306, 0.0103-0.0620 mg/mL; the average recoveries of total flavonoids and polysaccharides were 95.84% and 96.12%, with RSD of 1.86% and 1.97%, respectively. The average content of gallic acid, geniposide, paeoniflorin, narirutin, hesperidin were about 0.56%, 1.94%, 2.37%, 0.22%, 0.89%; the linear ranges of them were 6.03-301.27, 20.63-773.72, 20.18-504.41, 5.95-297.53, 9.90-247.62 μg/mL; the average recoveries of them were 97.54%, 102.39%, 96.48%, 103.60%, 98.22%, with RSD of 1.72%, 1.41%, 1.38%, 1.21%, 0.97%, respectively. Conclusion The established method for the content determination of total flavones, polysaccharides, inorganic salts and five active components has good accuracy and repeatability, which can provide comprehensive basis for the quality control of Huagan Decoction.

32 Research progress in biofilm prevention and treatment of Staphylococcus aureus

Staphylococcus aureus (SA) has caused many community-acquired and hospital-acquired infections around the world. The formation of biofilm is a kind of survival strategy for bacteria to adapt to their living environment. Under the protection of biofilm, bacteria in biofilm have become tolerant and resistant to antibiotics and the immune responses, making it difficult to treat infections associated with biofilm. Therefore, the prevention and treatment of SA biofilm has always been a research focus. This review summarizes the latest prevention and treatment strategies to eliminate SA biofilm formation and related infections.

36 Biological mechanism progross of anti-epileptic effect of Ganoderma

Recurrent seizures of epilepsy are harmful to human health. Ganoderma mainly contains triterpenoids, polysaccharides, sterols, alkaloids, peptides and amino acids. Its antiepileptic effects can be manifested by inhibiting neuronal apoptosis, reducing nerve excitability, antioxidant stress, inhibiting the release of inflammatory factors, regulating the balance of neurotransmitters, promoting neuronal repair and synaptic remodeling, and improving learning, memory and cognitive functions, so protect nerve cells and inhibit seizure. The development of traditional antiepileptic drugs combined with traditional Chinese medicine can learn from each other’s strong points to make them more effective, more tolerable and safer, so as to promote the progress of epilepsy treatment, so that the antiepileptic effect is stronger, more tolerable and safer, to promote the progress of epilepsy treatment.

40 Research progress of exosomes in rebuilding injured muscles

Skeletal muscle injury is a common clinical injury disease. The current treatment include traditional Chinese medicine and modern Western medicine, but these therapies have many problems. For example, massage and acupuncture have long treatment with poor efficacy, acupoint injection is easy to damage nerves, growth factor treatment can cause tumor-like malformations of muscle, stem cell transplantation may cause immune rejection, electrical stimulation results in mucosal injury and bleeding with poor durability. Exosomes are tiny vesicles rich in genetic substances and proteins, which play an important role in rebuilding injured muscles. As a novel therapy, exosomes can avoid the complications of the current treatments. Therefore, exosomes have broad prospects. This article reviews the current research progress of exosomes in rebuilding injured muscles.

44 Research progress of exosomes in breast cancer

Breast cancer cells leave the primary tissue and enter the circulatory system, along the circulatory system to reach the target organs, and eventually form distal metastasis and recurrence of breast cancer, which is the main cause of death of breast cancer patients. Exosomes are small lipid bilaterals released by pluripotent cells, which regulate complex cellular pathways and are involved in all stages of breast cancer from the onset to the spread of metastasis. Studies have shown that exosomes are closely related to breast cancer metastasis, which can promote tumor angiogenesis, help tumor escape the monitoring of the immune system and inhibit immune cells. In this review, we will discuss the role of exosomes in the occurrence and development of breast cancer from the structure of exosomes, and reveal whether exosomes can provide new prospects for the early diagnosis and molecular targeted therapy of breast cancer.

48 Progress in studies on the correlation between intestinal flora and the pathogenesis of bipolar disorder

Bipolar disorder is a major cause of economic burden and disability worldwide, but its underlying pathogenesis is still poorly understood, treatment is less effective and relapse is more likely. In recent years, more and more attention has been paid to the role of intestinal flora in bipolar disorder. The intestinal flora of patients with bipolar disorder changes, and it has two-way communicate with the brain through a variety of pathways, including immune inflammatory response, neuroendocrinology and nervous system, causing mood, energy levels and behavioral changes. Therefore, this paper reviews the research progress on intestinal flora and the pathogenesis of bipolar disorder, and provides new ideas for exploring the etiology, diagnosis, differential diagnosis and treatment of bipolar disorder.

52 Medical psychology network teaching feedback and its influence on medical students’ emotional intelligence

Objective To investigate and analyze the current status of medical psychology online teaching methods for medical students. Methods From March to July 2020, a total of 116 undergraduates from the 2018 traditional chinese medicine internal medicine class of Tianjin University of Traditional Chinese Medicine who took elective medical psychology courses were surveyed, and an electronic questionnaire was distributed. Soliciting suggestions on the effect of medical psychology online teaching methods and the selection of teaching methods. Results A total of 85 medical students completed all the research contents and were eventually included in the study, with a completion rate of 73.3%. All of them had studied medical psychology online, and 58.8% of medical students believe that blended teaching combining classroom and internet was their preferred teaching method. The trait coping style questionnaire showed that 49.4% of medical students could basically or certainly quickly turn negative factors into positive factors, while only 11.8% of students cannot quickly adapt to difficulties and pains. The emotional intelligence scale questionnaire showed that 56.5% of medical students were good at controlling their emotions, which was very important in the process of clinical admissions. Conclusion The blended teaching model combining medical psychology classrooms and the Internet is more accepted by students. To a certain extent, it promotes active responses and emotional management of medical students, thereby improving the humanistic quality of medical students and promoting the development of medical education.

56 Qualitative study on the demand for “Internet+” pulmonary rehabilitation in elderly patients after lung cancer surgery

Objective To understand the real needs of elderly patients after lung cancer surgery for “Internet+” pulmonary rehabilitation after discharge, and to provide references for the construction of home-based pulmonary rehabilitation programs for elderly lung cancer patients. Methods From October to November 2020, phenomenological research methods were used to conduct semi-structured personal in-depth interviews with 13 elderly postoperative lung cancer patients, and the Colaizzi content analysis method was used to analyze the data and refine themes. Results The needs of elderly patients after lung cancer surgery for pulmonary rehabilitation under the “Internet+” model were summarized into four themes: pulmonary rehabilitation health education, exercise therapy, homogeneous rehabilitation medical service resources, and the need for public welfare economic support. Conclusion Elderly patients after lung cancer surgery have a particularly prominent need for “Internet+” pulmonary rehabilitation, especially in rural areas where medical resources are scarce. A pulmonary rehabilitation program based on “Internet+” should be constructed and implemented in a timely manner to achieve homogenized medical, nursing, and rehabilitation services, and improve the quality of life of patients.

60 Analysis of the status quo and influencing factors of primary doctors’ professional identity

Objective To analyze the status quo and influencing factors of the professional identity of grassroots doctors in China under the background of the new medical reform, and to explore the strategies to improve the professional identity of grassroots doctors. Methods From November to December 2019, a questionnaire survey method was adopted to survey medical staff in some community health service centers and township health centers in Beijing, Guizhou Province, He’nan Province, and Jiangsu Province based on a self-designed occupational identity questionnaire. The occupational identity scores of the included people were divided into four groups by quartiles, and the influencing factors were analyzed. Results A total of 3 500 questionnaires were distributed, 3408 valid questionnaires were returned, and the effective response rate was 97%. The total score of professional identity was (25.87±3.84) points. There were statistically significant differences in gender, age, educational background, working years, and the proportion of working provinces among different occupational identity score groups (P < 0.05). Female (OR = 0.766), undergraduate (OR = 0.666), graduate and above (OR = 0.580), Guizhou Province (OR = 1.208), Jiangsu Province (OR = 1.705) were related factors that affect professional identity (P < 0.05). Conclusion The professional identity of grassroots doctors needs to be improved. Policy guidance should be further strengthened and the cultivation of personal values should be emphasized.

64 Analysis of the clinical value of finding the lower parathyroid glands through the pleural ligament

Objective To explore the application value of finding the lower parathyroid gland (LPG) through the pleural ligament. Methods A total of 110 patients with thyroid lesions undergoing surgical treatment who were admitted to Pizhou Hospital of Traditional Chinese Medicine from August 2018 to August 2020 were selected as the research objects. They were divided into group A (55 cases, thyroid surgery using fine capsule anatomy) and group B (55 cases, on the basis of group A, another LPG was searched through the pectoral ligament) according to a random number table. The LPG search conditions in the two groups were compared during the operation. The anatomical structure of the pleural ligament and the location of the LPG were recorded. The parathyroid function, inflammatory indexes and ostoperative complicationsof the two groups were compared before and after the operation. Results The proportion of people finding LPG in group B was higher than that in group A, the difference was statistically significant (P < 0.05). The pleura ligament group B was mostly a narrow strip of fat connective tissue structure with a narrow upper and lower width. Among the 43 patients, the end point was located at the lower 1/3 of the dorsal position of the lower pole of the thyroid, and the ligament contains adipose tissue and blood vessels. Among the 55 patients in group B, 21 cases of the LPG was located around the thyroid stop of the pleural ligament, 13 cases of the LPG was located in the pleural ligament, 2 cases of the LPG were located in the neck of the thymus, and 9 cases of the LPG was located around the pleural ligament. The preoperative parathyroid hormone (PTH), C-reactive protein (CRP), and blood Ca2+ levels were compared in the two groups before surgery, the difference was not statistically significant (P > 0.05). The PTH and blood Ca2+ levels at 1 day after operation in the two groups were lower than before operation, the differences were statistically significant (P < 0.05), and the PTH and blood Ca2+ levels at 1 day after operation in group B were higher than group A, the differences were statistically significant (P < 0.05). The level of CRP at 1 day after operation in the two groups was higher than before operation, the difference was statistically significant (P < 0.05), and the level of CRP at 1 day after operation in group B was lower than group A, the difference was statistically significant (P < 0.05). The overall incidence of complications in group B was lower than that in group A, the difference was statistically significant (P < 0.05). Conclusion Searching for LPG through the pectoral ligament can effectively locate and retain LPG, reduce the level of inflammation, and reduce the occurrence of postoperative complications.

68 A randomized controlled trial Polyethylene Glycol Loxenatide Injection for patients with type 2 diabetes mellitus

Objective To investigate the efficacy and safety of Polyethylene Glycol Loxenatide Injection (PEX-168) injection in the treatment of type 2 diabetes. Methods A total of 60 patients with type 2 diabetes mellitus in our hospital from August 2018 to August 2019 were randomly divided into two groups, they were divided into the L group and C group according to the random number table, with 30 cases in each group. Group C was given Metformin tablet 0.5 g/time, twice a day; Acarbose 25 mg/time, 2-3 times/day. Patients in group L were subcutaneously injected with PEX-168 0.5 ml/dose in the upper arm once a week for 12 weeks on the basis of group C during the treatment period. Four weeks after the end of the treatment period, fasting blood glucose, 2 h postprandial blood glucose and hemoglobin ALC (HbA1c) and bady mass index(BMI) were measured. The occurrence of adverse reactions such as hypoglycemia, nausea, vomiting, abdominal discomfort and headache in the two groups were recorded. Results The fasting blood glucose, 2 h postprandial blood glucose, HbA1c and BMI of the L group were significantly lower after treatment than before, with statistically significant differences (P < 0.05). Fasting blood glucose, 2 h postprandial glucose, HbA1c and BMI in the L group were significantly lower than those in the C group after treatment, with statistically significant differences (P < 0.05). After treatment, the number of patients in group L with HbA1c < 7.0% was significantly more than that in group C, with a statistically significant difference (P < 0.05). There was no significant difference in the incidence of hypoglycemia, headache, vomiting and abdominal discomfort between the two groups (P > 0.05). Conclusion PEX-168 Injections has high efficacy and safety in the treatment of type 2 diabetes, but the gastrointestinal reactions of patients should be observed.

72 Clinical effect of Kelintong Capsules combined with Tamsulosin Hydrochloride Sustained-release Capsules in the treatment of lower ureteral calculi

Objective To observe the clinical effect of Kelintong Capsules combined with Tamsulosin Hydrochloride Sustained-release Capsules in the treatment of lower ureteral calculi. Methods A total of 72 patients with lower ureteral calculi treated in Beijing First Hospital of Integrated Traditional Chinese and Western Medicine from July 2018 to June 2020 were selected as the research objects. The random number table method was used for grouping, and they were divided into treatment group and control group at a ratio of 1∶1, with 36 cases in each. The treatment group was treated with Kelintong Capsules combined with Tamsulosin Hydrochloride Sustained-release Capsules, and the control group was treated with Kelintong Capsules mimic combined with Tamsulosins Hydrochloride Sustained-release Capsules. The effects of Kelintong Capsules combined with Tamsulosin Hydrochloride Sustained-release Capsules in promoting stone removal, the number of days required for stone removal, reducing secondary lithotripsy, reducing renal colic, reducing the use of antibiotics and analgesics, and improving the clinical symptoms of traditional Chinese medicine were observed. Results During the treatment, four cases fell out, two cases in each group. The clinical treatment efficiency of the treatment group was higher than that of the control group, and the difference was statistically significant (P < 0.05). The need for a second lithotripsy in the treatment group was lower than that in the control group, and the difference was statistically significant (P < 0.05). The number of days required for stone removal and antibiotic use for the treatment group was shorter than that in the control group, and the difference was statistically significant (P < 0.05). There was no significant difference in the onset of renal colic between the two groups (P > 0.05). There was a statistically significant difference in the use of analgesics between the two groups (P < 0.05). After treatment, the scores of waist, abdomen and perineum pain, urination, hematuria, renal percussive pain and ureteral tenderness were lower in the two groups than before treatment, and the treatment group was lower than the control group, and the differences were statistically significant (P < 0.05). Conclusion After extracorporeal shock wave lithotripsy for patients with lower ureteral calculi, the use of Kelintong Capsules combined with Tamsulosin Hydrochloride Sustained-release Capsules for adjuvant treatment can significantly improve the clinical treatment effect of the patients, promote stone removal, quickly improve clinical symptoms, and reduce the incidence of postoperative complications.

76 Risk factors of contrast nephropathy after percutaneous coronary intervention and the diagnostic value of serum soluble Klotho protein

Objective To analyze the risk factors of contrast induced nephropathy (CIN) after percutaneous coronary intervention (PCI), and to analyze the diagnostic value of serum soluble Klotho protein for CIN. Methods A retrospective analysis of the clinical data of 143 patients undergoing PCI surgery at Suzhou Integrated Traditional Chinese and Western Medicine Hospital from May 2017 to June 2019. According to whether CIN occurred after operation, 143 patients were divided into CIN group (27 cases) and non-CIN group (116 cases). Logistic regression was used to analyze the risk factors for CIN, and the receiver operating curve (ROC) was used to evaluate the value of Klotho protein in the diagnosis of CIN. Results The incidence of CIN after PCI was 18.89%. CIN combination and diabetes ratio, systolic blood pressure, fasting blood glucose (FBG), serum creatinine (Scr), uric acid (UA), cystatin C (CysC), high sensitivity C-reactive protein (CRP), homocysteine (Hcy) were higher than the non-CIN group (P < 0.05). The estimated glomerular tiltration rate (eGFR) and Klotho protein levels were lower than those in the non-CIN group (P < 0.05). Logistic regression analysis showed that Scr (OR = 1.017), eGFR (OR = 3.443) and Klotho protein (OR = 2.855) were risk factors for CIN (P < 0.05). Pearson analysis showed that Klotho protein was negatively correlated with Scr (r = -0.32, P < 0.05), and positively correlated with eGFR (r = 0.45, P < 0.05). The area under the curve of Klotho protein in the diagnosis of CIN was 0.823, the sensitivity was 87.2%, the specificity was 74.5%, and the Youden index was 0.617. Conclusion Scr, eGFR and Klotho protein are the risk factors for CIN, and Klotho protein has good diagnostic value for CIN.

80 Expression of human cartilage glycoprotein-39 in serum and urine of patients with renal tuberculosis and its diagnostic and prognostic value

Objective To explore the expression level of human cartilage glycoprotein-39 in serum and urine of patients with renal tuberculosis and its diagnostic and prognostic value. Methods A total of 80 patients with renal tuberculosis admitted to the Department of Urology, the First Affiliated Hospital of Hebei North University (“our hospital” for short) from May 2019 to May 2020 were selected as the observation group, and 50 healthy people in our hospital were selected as the control group. Enzyme-linked immunosorbent assay was used to detect the concentration of human cartilage glycoprotein-39 in the serum and urine of the two groups, and the expression level of human cartilage glycoprotein-39 in serum and urine of renal tuberculosis patients and its diagnostic and prognostic value were explored. Results The levels of human cartilage glycoprotein-39 in urine and serum specimens of the observation group were significantly higher than those of the control group, and the differences were statistically significant (P < 0.05). The expression level of human cartilage glycoprotein-39 in renal tuberculosis lesion tissue was significantly higher than that in normal tissues around the week, and the difference was statistically significant (P < 0.05). The area under the ROC curve of human cartilage glycoprotein-39 was larger than that of PCR, and the difference was highly statistically significant (P < 0.01). Conclusion The level of human cartilage glycoprotein-39 in the urine and serum of patients with renal tuberculosis is significantly higher than that of normal people. The preliminary diagnosis of renal tuberculosis can be made by understanding the level of human cartilage glycoprotein-39 in the patient’s body.

84 Clinical effect of reconstructing irreducible comminuted radial head fracture with autogenous iliac bone graft

Objective To investigate the clinical effect of reconstructing irreducible comminuted radial head fracture with autogenous iliac bone graft. Methods A total of 27 patients with Mason type Ⅲ and type Ⅳ comminuted radial head fracture admitted to the Fourth Affiliated Hospital of Kunming Medical University from June 2016 to June 2019 were selected as the research subjects. The comminuted radial head fractures were all surgically reconstructed with autogenous iliac bone. All patients were followed up for more than 12 months. Heal, Broberg-Morrey elbow fracture outcome score, and disability of arm, shoulder and hand scale (DASH) score were recorded at the last follow-up. The range of motion of the elbow of the affected side and the healthy side was compared in the same patient at the last follow-up. Results At the last follow-up, X-ray graphs showed that 25 patients had bony healing of the radial head. The bony healing rate was 92.6%, and the mean healing time was (11.35±1.15) weeks. At the last follow-up, the excellent and good rate of Broberg-Morrey elbow fracture outcome score was 88.9%, and the average DASH scale score was (15.52±4.39) points. At the last follow-up, the flexion and extension, pronation and supination range of motion of the elbow of the affected side was lower than that of the healthy side, and the differences were statistically significant (all P < 0.05). Conclusion Reconstruction of irreducible comminuted radial head fractures with autogenous iliac bone graft can achieve satisfactory results in the short and medium term, but the long-term results need to be further studied.

88 Effect of fibular allograft transplantation on prevention of inversion and sag after comminuted proximal humerus fracture operation in the elderly

Objective To study the effect of fibular allograft transplantation combined with locking plate on prevention of inversion and sag after comminuted proximal humerus fracture operation in the elderly. Methods Clinical data of 72 elderly patients with comminuted proximal humerus fractures admitted to Beijing Jishuitan Hospital from January 2015 to December 2018 were summarized. The patients were divided into two groups according to the operative methods: the plate group (41 cases) treated with locking plate alone, and the graft group (31 cases) treated with fibular allograft transplantation combined with locking plate. The operative time and intraoperative blood loss were compared between the two groups. The follow-up time, visual analogue scale (VAS) at last follow-up, University of California, Los Angeles (UCLA) shoulder function score and Constant shoulder score were compared between the two groups. The changes of neck-shaft angle and humeral head height were compared between the two groups after operation and the last follow-up. Results There were no significant differences in operative time, intraoperative blood loss, follow-up time and VAS at the last follow-up between the two groups (P > 0.05). UCLA shoulder function score and Constant shoulder score in the graft group were higher than those in the plate group at the last follow-up, with statistical significance (all P < 0.05). The neck-shaft angle and humeral head height in the last follow-up in both groups were lower than those after operation, and those in the graft group were higher than those in the plate group, with statistical significance (all P < 0.05). Conclusion Fibular allograft transplantation combined with locking plate in the treatment of comminuted proximal humerus fractures in elderly has a good effect of inversion and sag of the humerus head, and can improve the prognosis of patients.

92 Effect of unilateral PVP on vertebral height recovery and the degree of pain score after surgery in patients with OVCF

Objective To investigate the recovery of vertebral height and the degree of pain after unilateral percutaneous vertebroplasty (PVP) for osteoporotic vertebral compression fracture (OVCF). Methods A total of 84 patients with thoracolumbar single vertebral body OVCF who received PVP treatment in Taizhou Second People’s Hospital, Jiangsu Province from September 2016 to September 2019 were selected. According to the surgical approach, they were divided into unilateral group and bilateral group, including 48 cases in unilateral group and 36 cases in bilateral group. The surgical time, surgical fluoroscopy times and the amount of bone cement injection were compared between the two groups. The changes of visual analogue score (VAS), Oswestry disability index (ODI), vertebral height and Cobb angle were compared between the two groups before and three days after surgery and one year after surgery. Results The times of surgical X-ray fluoroscopy in unilateral group was less than that in bilateral group, and the amount of bone cement injection was lower than that in bilateral group, with statistical significance (all P < 0.05). There were statistically significant differences in VAS and ODI scores between the two groups before and three days after surgery, and one year after surgery, as well as at time points (P < 0.05). Further pairwise comparison showed that VAS and ODI scores at three days and one year after surgery were lower than those before surgery, and one year after surgery were lower than those at three days after surgery, with statistical significance (all P < 0.05). VAS and ODI scores of the unilateral group were lower than those of the bilateral group at the same time point, and the differences were statistically significant (all P < 0.05). There were statistically significant differences in the time point comparison and interaction of vertebral height, Cobb angle between the two groups before and three days and one year after surgery (P < 0.05). Further pairwise comparison: the vertebral height at three days and one year after surgery in both groups was higher than before surgery, Cobb angle was lower than before surgery, and the vertebral height at one year after surgery was lower than three days after surgery, Cobb angle was greater than three days after surgery, the differences were statistically significant (all P < 0.05). Conclusion Both unilateral and bilateral PVP were used in the treatment of OVCF, and the unilateral approach was more advantageous than the bilateral approach in terms of surgry time and number of X-ray fluoroscopy.

96 Clinical effect of short puncture combined with blood-letting puncture in the treament of knee osteoarthritis

Objective To observe the clinical effect of short puncture combined with blood-letting puncture in the treament of knee osteoarthritis. Methods A total of 106 patients with knee osteoarthritis admitted to the outpatient service of Department of Pricking Blood, Hefei First People’s Hospital from January 2018 to October 2020 were selected. They were divided into observation group (53 cases) and control group (53 cases) by random number table method. The observation group was treated with short puncture combined with blood-letting puncture, while the control group was treated with conventional acupuncture combined with blood-letting puncture. The clinical effect, visual analogue score (VAS) and Japanese orthopedic association (JOA) evaluate treatment score of the two groups were observed after four weeks of treatment. Results There was statistically significant difference in clinical efficacy between the two groups (P < 0.05). After treatment, the VAS in both groups were lower than those before treatment, the score of each dimension and the total score of JOA in both groups were higher than those before treatment, and the VAS in the observation group was lower than that in the control group, and the score of each dimension and the total score of JOA in the observation group were higher than those in the control group, with statistical significance (all P < 0.05). Conclusion Short puncture combined with blood-letting puncture in the treatment of knee osteoarthritis has significant advantages, can effectively improve the symptoms of knee osteoarthritis, worthy of promotion.

100 Characteristics of the part of bony junction after distal tibiofibular fracture and its effect on ankle joint function

Objective To investigate the characteristics of bony junction after distal tibiofibular fracture and its effect on ankle joint function. Methods The clinical data and imaging data of 74 patients with bony junction after distal tibiofibular fracture in Xuzhou Renci Hospital, Jiangsu Province from February 2014 to October 2018 were retrospectively analyzed. The imaging data and the incidence of complications were analyzed, and the range of motion of the affected and healthy ankles and the American orthopedic foot ankle society (AOFAS) score were compared at the last follow-up. Results Among the 53 distal tibiofibular fractures involving the ankle joint, 48 cases had bony junction at the tibiofibular syndesmosis level. Eleven of the 14 cases of simple distal tibionfibular fractures had bony junction at the fracture line, two of the four cases of tibia shaft combined with lateral malleolus fractures had bony junction at the locking nail at the distal end of the tibial intramedullary needle, and three cases of Pilon fracture had bony junction at the fracture line. At the last follow-up, the range of motion of plantar flexion, active dorsal extension, passive dorsal extension and AOFAS score of the affected side were lower than those of the healthy side, with statistical significance (all P < 0.05). Adverse reactions occurred in 14 patients, including displacement of the broken end of the screw, needle tract infection, reduced range of motion or signs of osteoarthritis in the ankle, post-exertion soreness on the affected side, and slight discomfort in walking up and down stairs. Conclusion The bony junction of distal tibiofibular fracture occurs mostly in the fracture line, and the range of motion of the affected side of the ankle is lower than that of the healthy side, but it does not affect the daily life and work.

103 Prognostic effect of laparoscopic extensive hysterectomy combined with pelvic lymph node resection on early cervical cancer and the influence of pelvic floor muscle function

Objective To explore the prognostic effect of laparoscopic extensive hysterectomy combined with pelvic lymph node resection on early cervical cancer and the influence of pelvic floor muscle function. Methods Retrospective analysis was made on the clinical data of 80 patients with early cervical cancer who underwent surgical treatment were selected in Department of Gynecology and Obstetrics, Bozhou Hospital of Traditional Chinese Medicine, Anhui Province from December 2016 to December 2019. According to surgical methods, they were divided into observation group and control group, with 40 cases in each group. Observation group was treated with laparoscopic extensive hysterectomy combined with pelvic lymph node resection, while control group was treated with conventional laparotomy extensive hysterectomy combined with pelvic lymph node resection. After the operation, the patients were followed up, the functional assessment of cancer therapy-general (FACT-G) score, half a year and one year survival rate after surgery, recurrence rate within one year after surgery, normal rate of pelvic floor muscle strength, female sexual function index (FSFI) score and incidence of sexual dysfunction were compared between two groups. Results One year after surgery, FACT-G score of observation group was higher than that of control group, and the difference was highly statistically significant (P < 0.01). There were no significant differences between two groups in half a year, one year survival rate after surgery, and recurrence rate within one year after surgery (P > 0.05). The normal rate of pelvic floor muscle strength and FSFI score in observation group were higher than those in control group, and the differences were statistically significant (P < 0.05 or P < 0.01); the incidence of sexual dysfunction was lower than that of control group, and the difference was highly statistically significant (P < 0.01). Conclusion Laparoscopic extensive hysterectomy combined with pelvic lymph node resection in the treatment of early cervical cancer has the same prognosis as laparotomy. The patients’ quality of life is significantly improved, and the pelvic floor function is well recovered after operation, which is worthy of clinical application.

107 Effect of integrated traditional Chinese and Western medicine on early threatened abortion and pregnancy outcome

Objective To investigate the effect of integrated traditional Chinese and Western medicine on early threatened abortion and the effect of pregnancy outcome. Methods A total of 118 patients with threatened abortion treated in Liaocheng Traditional Chinese Medicine Hospital from January 2018 to January 2020 were selected as study objects. According to the random number table method, they were divided into control group and observation group, with 59 cases in each group. Control group was treated with conventional Western medicine, and observation group was treated with Jiawei Shoutai Pills on the basis of control group, and the treatment lasted for 2 courses. The clinical curative effect, traditional Chinese medicine syndrome score, serum hormone level, complication and pregnancy outcome were compared between two groups. Results The total effective rate of observation group was higher than that of control group, and the difference was statistically significant (P < 0.05). Before treatment, there was no significant difference in traditional Chinese medicine syndrome score between two groups (P > 0.05). After treatment, traditional Chinese medicine syndrome scores of two groups were lower than those before treatment, and observation group was lower than control group, and the differences were statistically significant (P < 0.05). Before treatment, there were no significant differences in serum levels of human chorionic gonadotrophin-β subunit (β-HCG), estradiol (E2), and progesterone (P) between two groups (P > 0.05). After treatment, the levels of β-HCG, E2, and P in two groups were higher than those before treatment, and observation group was higher than control group, and the differences were statistically significant (P < 0.05). There was no significant difference in the total incidence of complications between two groups (P > 0.05). The birth rate, newborn weight and Apgar score of observation group were higher than those of control group, and the neonatal asphyxia rate and premature birth rate were lower than those of control group, and the differences were statistically significant (P < 0.05). Conclusion The clinical effect of the treatment of early threatened abortion patients with integrated traditional Chinese and Western medicine is significant, can effectively alleviate the clinical symptoms, regulate the serum hormone level, while not increasing the incidence of complications, to obtain a better pregnancy outcome.

111 Effect of vaginal microecology changes on premature rupture of membranes and maternal and fetal outcomes

Objective To investigate the effect of vaginal microecology changes on premature rupture of membranes (PROM) and maternal and fetal outcomes. Methods Pregnant women with PROM who delivered in Huai’an College of Women and Children Clinical, Xuzhou Medical University (hereinafter referred to as “our hospital”) from September 2019 to February 2020 were selected as research objects. According to the different periods of PROM, they were divided into term PROM (tPROM) 32 cases and preterm PROM (PPROM) 48 cases, 32 healthy pregnant women in our hospital during the same period were selected as control group. The detection of vaginal secretions and vaginal microflora among three groups were compared; the incidence of pathological jaundice, puerperal infection rate, neonatal blood gas pH value and Apgar 1 min score were compared among three groups. Results The positive rates of acetylglucosaminidase, catalase, β-glucuronidase, leucocyte lipase and the ratio of pH > 4.5 in PPROM group were higher than those in control group, and the differences were statistically significant (P < 0.017). The positive rates of acetylglucosaminidase, catalase, sialidase, leucocyte lipase and the ratio of pH > 4.5 in tPROM group were higher than those in control group, and the differences were statistically significant (P < 0.017). The infection rates of bacterial vaginitis and chlamydia trachomatis in PPROM group were higher than those in tPROM group and control group; the abnormal rate of vaginal secretions and the infection rate of vulvovaginal candidiasis were higher than those in control group, and the differences were statistically significant (P < 0.017). The abnormal rate of vaginal secretions and ureaplasma urealyticum infection in tPROM group were higher than those in control group, and the differences were statistically significant (P < 0.017). The rate of cesarean section, incidence of pathological jaundice and neonatal blood gas pH in PPROM group were higher than those in tPROM group and control group, and Apgar 1 min was lower than that in tPROM group and control group, and the differences were statistically significant (P < 0.017 or P < 0.05). Conclusion Changes in vaginal microenvironment during pregnancy and reproductive tract infection are the pathogenic factors of PROM, and changes in vaginal microenvironment affect maternal and fetal outcomes.

115 Evaluation of risk factors of venous thromboembolism in gynecological patients and construction of risk early warning scale

Objective To explore the risk factors of postoperative venous thromboembolism (VTE) in gynecological patients, and to construct and verify the risk early warning scale of gynecological VTE. Methods Clinical data of patients who underwent gynecological surgery in Shanghai Fengxian District Central Hospital from January 2017 to December 2019 were retrospectively analyzed. Fifty-three patients with VTE were selected as the case group, and 167 patients without VTE were selected as the control group according to the ratio of 1∶3 in the same department and hospitalized at the same time. The risk factors of postoperative VTE in gynecological patients were analyzed, and the risk early warning scale of gynecological VTE was constructed and verified. Results There were statistically significant differences between the two groups in the proportion of age, hypertension, hyperlipidemia, intraoperative blood transfusion, operative time, bed time, disease types and use of contraceptives (P < 0.05). Age≥60 years old, hypertension, use of contraceptives, hyperlipidemia, operation time ≥180 min, bed time ≥96 h, malignant tumor were risk factors for VTE (OR >1, P < 0.05). The standard of gynecological VTE risk early warning scale constructed was low risk < 2.8 points and high risk ≥2.8 points. There was statistical significance in the proportion of different risk grades between the two groups (P < 0.05). Conclusion The risk factors of gynecological patients complicated with VTE are complex. The risk early warning scale can be used to prevent and reduce the incidence of VTE in gynecological surgery patients, and is easy to be applied in clinical practice.

119 Effect of anterior chamber depth before operation on refractive status in patients with age-related cataract after phacoemulsification

Objective To explore the effect of anterior chamber depth (ACD) before operation on refractive status in patients with age-related cataract after phacoemulsification. Methods A total of 123 patients with age-related cataract (123 eyes) in Xinjiang Uygur Autonomous Region People’s Hospital from April 2018 to April 2019 were collected as the research subjects. According to preoperative ACD, they were divided into high risk shallow anterior chamber group (35 cases), mild shallow anterior chamber group (46 cases), and normal anterior chamber group (42 cases). The parameters of diopter, ACD and so on were determined before, one day after and three months after operation. The correlation between ACD and refractive error was analyzed by Pearson product moment correlation. Results There were statistically significant differences in ACD among different groups and at different time points (P < 0.05). Preoperative ACD of the high risk shallow anterior chamber group and the mild and shallow anterior chamber group were lower than those of the normal anterior chamber group, and the high and shallow anterior chamber group was lower than that of the mild shallow anterior chamber group (P < 0.05). ACD of patients in the high and shallow anterior chamber group and the mild shallow anterior chamber group increased one day and three months after surgery compared with that before operation (P < 0.05). There was no difference in refractive error among the three groups at one day after operation (P > 0.05). Three months after operation, the refractive error of the high and shallow anterior chamber group and the mild and shallow anterior chamber group was greater than that of the normal anterior chamber group, and the high and shallow anterior chamber group was greater than that of the mild and shallow anterior chamber group (P < 0.05). According to Pearson product moment correlation analysis, preoperative ACD of age-related cataract patients was negatively correlated with the refractive error three months after surgery (r = -0.759, P < 0.05). Conclusion ACD is negatively associated with refractive error in patients with age-related cataract after phacoemulsification. Accurate preoperative evaluation of ACD has a great significance for clinical operation plan and improvement of postoperative diopter.

123 Levels and clinical significance of TGF-β1 and TGF-β2 in patients with diabetic retinopathy

Objective To investigate the serum levels of transforming growth factor (TGF) -β1 and TGF-β2 and their clinical significance in diabetic retinopathy (DR) patients. Methods A total of 189 patients with type 2 diabetes mellitus who were treated in Branch Hospital of Huai’an First People’s Hospital (“our hospital” for short) in January 2015 and May 2018 were selected, including 61 DR patients (DR group), 38 non-value-added diabetic retinopathy (NPDR) and 23 value-added diabetic retinopathy (PDR). There were 128 patients without DR (NDR group). At the same time, 100 health checkups from our hospital during the same period were selected as the control group. The levels of TGF-β1, TGF-β2, fasting blood glucose, and glycated hemoglobin (HbA1c) in serum were detected. Results Serum TGF-β1, TGF-β2 and HbA1c in DR group were higher than those in NDR group and control group (P < 0.05). Fasting blood glucose was higher than that in control group (P < 0.05). Serum TGF-β1, TGF-β2, and HbA1c in PDR patients were higher than those in NPDR patients (P < 0.05). There was no significant difference in fasting blood glucose between PDR and NPDR patients (P > 0.05). TGF-β1 and TGF-β2 were positively correlated with HbA1c (r = 0.347, 0.457, P < 0.05). Conclusion Serum TGF-β1 and TGF-β2 are significantly increased in DR patients, which may play an important role in the occurrence and development of the disease, and have a positive correlation with HbA1c level.

127 Clinical effect of Sertraline Hydrochloride Tablets combined with Risperidone in the treatment of residual schizophrenia

Objective To investigate the protective effect of Sertraline Hydrochloride Tablets combined with Risperidone on the brain of residual schizophrenia. Methods A total of 70 patients with residual schizophrenia who were diagnosed and treated in the Third People’s Hospital of Fuyang City, Anhui Province from June 2016 to June 2019 were selected and divided into two groups according to random number table method: the combined treatment group (35 cases, Sertralin Hydrochloride Tablet and Risperidone treatment) and the control group (35 cases, Risperidone treatment). The positive and negative symptom scale (PANSS) positive, negative and total scores and  the simple mental strength scale (MMSE), before treatment and six months and 12 months after treatment were compared. Results Six months after treatment, PANSS positive, negative and total scores were lower in both groups than that before treatment, while MMSE score was higher than that before treatment (P < 0.05). At 12 months after treatment, PANSS positive, negative and total scores were lower than those before treatment and after six months of treatment, while MMSE score was higher than those before and after six months of treatment (P < 0.05). At six months after treatment, PANSS positive and total score in the combined treatment group were lower than those in the control group, while MMSE score was higher than that in the control group (P < 0.05). At 12 months after treatment, the positive, negative and total scores of PANSS in the combined treatment group were lower than those in the control group, while the MMSE score in the combined treatment group was higher than that in the control group (P < 0.05). Conclusion Sertraline Hydrochloride Tablets combined with Risperidone can effectively reduce PANSS score and increase MMSE score in patients with residual schizophrenia, and have a brain protective effect on patients with residual schizophrenia.

131 Research on medication rule of modern doctors in the treatment of Alzheimer disease based on data mining

Objective To collect and sort out the prescription rules of traditional Chinese medicine in the prevention and treatment of Alzheimer disease. Methods The randomized controlled trials of Alzheimer disease were searched in CNKI, VIP database and Wanfang database. The retrieval time was limited to October 2020. The literatures were screened, and then Apriori algorithm was used for statistical analysis. Results A total of 173 Chinese herbal compounds were screened out, involved 190 Chinese herbal medicines. The total frequency of Chinese herbal medicines was 1752, and 12 of them were more than 40. Traditional Chinese medicine for the prevention and treatment of Alzheimer disease is mostly tonifying deficiency drugs, promoting blood circulation and removing blood stasis drugs and phlegm, relieving cough and asthma drugs. Most of them belong to liver, kidney, lung and spleen meridians, with mild nature and sweet and bitter taste. The results of Apriori calculation showed that the compatibility of two herbs were Polygalae Radix and Acori Tatarinowii Rhizoma. The common compatibility of three herbs were Rehmanniae Radix Praeparata, Polygalae Radix and Acori Tatarinowii Rhizoma; Chuanxiong Rhizoma, Polygalae Radix and Acori Tatarinowii Rhizoma. The common compatibility of four herbs were Salviae Miltiorrhizae Radix et Rhizoma, Rehmanniae Radix Praeparata, Polygalae Radix and Acori Tatarinowii Rhizoma. Conclusion In this study, Polygalae Radix and Acori Tatarinowii Rhizoma have the highest correlation, and the prescription can be made based on the compatibility of this drug pair.

“Lifting the pot and removing the lid” is one of the analogistic treatment methods in Chinese medicine, mainly including two contents: in the narrow sense, invigorating the body and dispelling pathogenic factors to operate the stagnated lung qi; in a broad sense, it means that supporting the qi of five internal organs to regulate qi activity. IgA nephropathy is a common primary glomerular disease. The onset or acute exacerbation is often characterized by exogenous pathogenic factors invading lung and insecurity of the lung and the defensive qi, and chronic protracted stage is often characterized by dysfunction of qi movement. Therefore, for the reason that wind is the main exogenous pathogenic factor leading to the onset or acute exacerbation, it can be considered to treat IgA nephropathy by focusing on the lung, with Yupingfeng Power, Yinqiao Powder and other prescriptions to strengthen and protect lung, and to expel pathogenic factors. For patients with poor clinical efficacy of chronic delay or frequent relapse, the method of “lifting the pot and removing the lid” can be used. For patients whose lung qi are stagnated in the upper jiao, using Platycodonis Radix and bitter apricot seed can promote and regulate the lung qi. For those with disorder of ascending and descending of the spleen qi of the middle jiao, small dose of Cimicifugae Rhizoma and Bupleuri Radix can be added to raise yang. The exploration on the theoretic foundation of “lifting the pot and removing the lid” opens a new avenoue to clinical treatment of IgA nephropathy.

Professor Li Liangquan considers that chronic diseases are often deficient, and chronic diseases or complicated diseases are closely related to blood stasis and blood deficiency. Objective To explore the medication rules of traditional Chinese medicine in the treatment of vertigo by using traditional Chinese medicine inheritance auxiliary platform. Methods The related literatures on the treatment of vertigo with traditional Chinese medicine from May 2015 to May 2020 in CNKI were searched. The theme with “vertigo” or “dizziness” and “Chinese medicine” or “traditional Chinese medicine” was advanced searched. The prescription was screened and the database was built. The frequency of drug use and drug pairs use, association rules and prescription rules were analyzed. Results Finally, 234 prescriptions were included, involved 235 kinds of traditional Chinese medicine, and the high-frequency drugs were Atractylodis Macrocephalae Rhizoma, Poria, Gastrodiae Rhizoma, Chuanxiong Rhizoma and Angelicae Sinensis Radix. The most commonly used traditional Chinese medicines were tonifying deficiency drugs, diuretic drugs, calming liver and calming wind drugs, promoting blood circulation and removing blood stasis drugs, etc. In terms of drug properties, the four qi was mainly warm and cold, the five flavors was mainly sweet and pungent, and the meridian tropism was mainly liver and spleen meridians. High frequency drug combinations included “Atractylodis Macrocephalae Rhizoma and Poria” “Atractylodis Macrocephalae Rhizoma and Gastrodiae Rhizoma” “Poria and Gastrodiae Rhizoma”, etc. Among the association rules between drugs, “Citri Reticulatae Pericarpium -> Atractylodis Macrocephalae Rhizoma” had the strongest association. Twelve core combinations and six new prescriptions were found. Conclusion Through excavation and analysis, it is concluded that the treatment of vertigo mainly includes regulating qi and activating blood circulation, drying dampness and resolving phlegm, calming liver yang, tonifying qi and blood, nourishing yin and kidney, which is in line with the principle of treating both symptoms and signs of traditional Chinese medicine. Its core combination and new prescription can provide reference for clinical medication and new drug research and development.

But nowadays, the literature has few separate discussions on the prevention of microscopic disease. Therefore, through reading and analyzing the original text of Inner Canon of Yellow Emperor, the concept of “micro diseases” prevention and control is discussed. After sorting out the original text of Inner Canon of Yellow Emperor, it is known that the micro diseases is not serious, the healthy qi is slightly damaged, the disease position is shallow and the disease is light, and the symptoms are not obvious, which is easy to be ignored, but it can be turned into a serious illness, and it needs excellent medical skills to identify. On the basis of the five zang viscera, micro diseases can be divided into five categories, “Shenzhiwei”, “Baiqiweixie”, “Liuxue”, “Weifeng” and “Gujiedong”, which can be found in the vellus hair, skin texture, collaterals, muscles, joints and other parts. Its manifestations include abnormal body surface sensation, skin discomfort and micro sweat, blood stasis, abnormal movement of muscle and bone segments, etc. In the treatment, according to the characteristics and diseases of the five zang viscera, regulating qi and blood, unblocking collaterals and expelling pathogenic factors is taken as the principle. The use of medicine and food recuperation, acupuncture, guided massage, emotional counseling and other methods with the use of conditioning, so that the human body qi and blood, meridians unobtrusional, five zang viscera stability, micro diseases to recover, no longer trouble.

160 Effect of Guizhi Shaoyao Zhimu Decoction combined with Methotrexate in the treatment of cold-damp blockage syndrome of rheumatoid arthritis

Objective To investigate the effect of Guizhi Shaoyao Zhimu Decoction combined with Methotrexate in the treatment of cold-damp blockage syndrome of rheumatoid arthritis. Methods Eighty cases of cold-damp blockage syndrome of rheumatoid arthritis patients admitted to Lu’an People’s Hospital of Anhui Province from February 2017 to May 2020 were selected and they were divided into control group and study group according to random number table method, with 40 cases in each group. The control group was treated with Methotrexate, and the study group was treated with Guizhi Shaoyao Zhimu Decoction on the basis of the control group. After two months of treatment, the clinical efficacy and the incidence of adverse reactions of the two groups were compared, and the levels of inflammatory factors and the recovery of joint function were analyzed before and after two months of treatment. Results After two months of treatment, the total effective rate of the study group was higher than that of the control group, and the difference was statistically significant (P < 0.05). After two months of treatment, the levels of C-reactive protein (CRP), tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) in both groups were decreased compared with before treatment, and those in the study group were lower than those in the control group, with statistical significance (P < 0.05). After two months of treatment, the morning stiffness time, joint pain index, joint swelling number and joint tenderness number in both groups were lower than before treatment, and the study group was lower than the control group, the differences were statistically significant (P < 0.05). The total incidence of adverse reactions in the study group was lower than that in the control group, and the difference was statistically significant (P < 0.05). Conclusion Guizhi Shaoyao Zhimu Decoction combined with Methotrexate has a significant effect on cold-damp blockage syndrome of rheumatoid arthritis patients, and can significantly reduce the levels of CRP, TNF-α and IL-6, which is worthy of promotion and application.

Clinical akupotomye treatment of external humeral epicondylitis is mostly combined therapy, and the emergence of new akupotomye tools, such as super-micro akupotomye, water akupotomye, embedding akupotomye, etc. In this paper, the characteristics and clinical application of different akupotomye treatment are summarized and analyzed, so as to provide reference for the future research and development direction of akupotomye in the treatment of chronic strain diseases such as external humeral epicondylitis.

168 Clinical effect of small akupotomye in treatment of rheumatoid arthritis with cold-dampness blockage type

Objective To investigate the clinical effect of small akupotomye in the treatment of rheumatoid arthritis with cold-dampness blockage type. Methods A total of 65 patients with rheumatoid arthritis with cold-dampness blockage type admitted to First Affiliated Hospital of Xinjiang Medical University from March 2017 to December 2019 were selected as observation objects, and they were divided into observation group (32 cases) and control group (33 cases) according to random number table method. The control group was treated with conventional Western medicine, and the observation group was treated with small akupotomye on the basis of the control group. The treatment lasted for two weeks. Symptomatic changes, levels of rheumatoid factor (RF), oxidative stress related indexes and serum inflammatory factors were compared between the two groups before and after treatment. Results After treatment, joint swelling index and joint tenderness index of the two groups were lower than those before treatment, and the morning stiffness time was shorter than that before treatment, and the joint swelling index and joint tenderness index of the observation group were lower than those of the control group, and the morning stiffness time was shorter than that of the control group, with statistical significance (all P < 0.05). After treatment, the levels of RF and malondialdehyde (MDA) in two groups were lower than those before treatment, and the levels of superoxide dismutase (SOD) in two groups were higher than those before treatment, and the levels of RF and MDA in the observation group were lower than those in the control group, and SOD in observation group was higher than that in the control group, the differences were statistically significant (all P < 0.05). After treatment, the levels of interleukin (IL)-1, IL-6 and C-reactive protein in two groups were lower than those before treatment, and the observation group was lower than the control group, with statistical significance (all P < 0.05). Conclusion Small akupotomye treatment can significantly improve the clinical symptoms of rheumatoid arthritis patients with cold-dampness blockage type, reduce oxidative stress and inflammatory response, and reduce the level of RF.

172 Investigating the clinical effect of akupotomye in the treatment of knee osteoarthritis based on human bowstring mechanics theory

Objective To explore the clinical effect of akupotomye in the treatment of knee osteoarthritis (KOA) under the guidance of human bowstring mechanics theory. Methods Sixty patients with KOA who were admitted to Shijiazhuang Hospital of Traditional Chinese Medicine, Hebei Province from January to November 2019 were selected and they were divided into akupotomye group and acupuncture group by random number table method, with 30 patients in each group. The acupuncture group was given priority to the spleen channel and stomach channel acupoints through the knee joint, combined with local ashi points, acupuncture was performed continuously five times a week, once a day, and rest for two days, for a total of ten times. The akupotomye group was given priority to release the soft tissue adhesions and scars around the knee joint. The treatment was done once for seven days, and the treatment was done twice in total. Visual analogue scale (VAS) and the Western Ontario and McMaster Universities (WOMAC) osteoarthritis index score were measured before and after treatment, and the two groups were compared. The clinical efficacy of the two groups was compared after treatment. Results The VAS and WOMAC osteoarthritis  scores of the two groups showed no statistically significant difference before treatment (P > 0.05), but after treatment, the VAS and WOMAC osteoarthritis index scores of the two groups were lower than those before treatment, and the akupotomye group was lower than the acupuncture group, the differences were statistically significant (all P < 0.05). The total effective rate of akupotomye group was higher than that of acupuncture group, and the difference was statistically significant (P < 0.05). Conclusion The effect of human bowstring mechanics theory on akupotomye treatment of KOA is better than that of traditional acupuncture treatment, and it has the significance of popularizing. Its long-term efficacy needs further clinical verification.

Drug-induced liver injury is the most common and serious adverse drug reactions, a case of lung cancer with brain metastases developed severe drug-induced cholestasis after oral Anlotinib combined with Temozolomide chemotherapy. Liver injury continued to progress after routine liver preservation and cholestasis improvement therapy, and improved after treatment with glucocorticoid. According to the literature review, there have been many cases of pathologically confirmed severe cholestasis or even vanishingbile duct syndrome after the application of Temozolomide, a few cases it can lead to death. Timing glucocorticoid therapy and adjusting glucocorticoid dosage according to the therapeutic effect can reduce liver inflammation and cholestasis and improve the prognosis of patients.

This study narrates and synthesizes nine outcome frameworks, including World Health Organization’s definition of health, 5Ds, international classification of functioning, patient-reported outcomes measurement information system, Porter’s outcome hierarchy, health-related quality of life conceptual model, outcome measurement in rheumatdogy Filter 2.0 core set, Cochrane outcome framework, and Williamson/Clarke outcome, introduces and compare framework’ contents. It is concluded that the Williamson/Clarke outcome framework has a clear hierarchical structure and comprehensive content, which is suitable for the taxonomy of all disease outcomes, and clarify the consistency of outcome expression between studies. It should be recommended in clinical trials and practice.

189 Thoughts on the construction of traditional Chinese medicine management discipline group under the background of “double first-class” construction

Traditional Chinese medicine is an important part of medicine and pharmacy in China. The inheritance and innovation of Chinese medicine is a policy and management issue. How to make traditional Chinese medicine management run efficiently and develop sustainably still faces many difficulties and challenges. Based on the national “double first-class” strategy, this paper combs the development trend of the interdisciplinary under the “double first-class” construction. And on the basis of “double first-class” construction, the paper analyzes the current situation of the construction of Chinese medicine management disciplines. On this basis, some thoughts and trends analysis the construction of the construction of Chinese medicine management disciplines are put forward.

193 Discussion on optimization strategy of rotation training system for new hospital administrative staff based on satisfaction evaluation

Objective To provide satisfaction degree analysis and optimization strategy on rotation training of administrative staffs newly joined hospital. Methods The self-prepared satisfaction questionnaire was distributed to 13 newly recruited management personnel and 17 heads of administrative functional departments who took the job rotation from August 2019 to June 2020 in Cancer Hospital, Chinese Academy of Medical Sciences and Peking Union Medical College. The survey was carried out on the overall effect of job rotation training, cycle arrangement, department arrangement, staff ability improvement, post comprehensive ability evaluation and other aspects, and descriptive analysis was conducted on the statistical results. Results Rotating employees’ satisfaction with the overall effect of rotation training was 92.31%, and the satisfaction of rotation departments was 94.12%. The degree of satisfaction of rotation staff to rotation cycle was 92.3%, and the degree of satisfaction of rotation department was 70.58%. Rotation training is helpful for employees to get familiar with the environment and improve their ability. Conclusion It needs to strength post competency training, personalize rotation training plan, establish a suitable capability assessment mechanism, and ensure rotation training quality and effect.
