Abstract Drug-induced liver injury is the most common and serious adverse drug reactions, a case of lung cancer with brain metastases developed severe drug-induced cholestasis after oral Anlotinib combined with Temozolomide chemotherapy. Liver injury continued to progress after routine liver preservation and cholestasis improvement therapy, and improved after treatment with glucocorticoid. According to the literature review, there have been many cases of pathologically confirmed severe cholestasis or even vanishingbile duct syndrome after the application of Temozolomide, a few cases it can lead to death. Timing glucocorticoid therapy and adjusting glucocorticoid dosage according to the therapeutic effect can reduce liver inflammation and cholestasis and improve the prognosis of patients.
