Discussion on the construction of health emergency management training system

Public health emergencies are an important test for the ability of leading cadres to manage health emergencies at all levels. As the key force of health emergency management system, it is very important to set up a contingent of health emergency management cadres matching with the current development. Health emergency management training and system construction is an important way to improve the ability of health emergency management of leading cadres. Based on the present situation and training demand of health emergency management training in our country, drawing lessons from the beneficial experience of developed countries, this paper explores the construction of health emergency management training system guided by training demand, led by the Party committee, led by the government, coordinated by various units and responsible by training institutions, so as to provide reference and enlightenment for the development of health emergency management education and training in our country.

Analysis of the current situation and obstacle factors of telemedicine health poverty alleviation in the post-poverty era

Under the guidance of national policies and active exploration by various localities, telemedicine health poverty alleviation has continuously expanded the coverage area, enriched service content, and innovated medical models. By analyzing the current situation of the implementation of telemedicine health poverty alleviation in China, it is found that it faces obstacles such as capital investment, technical configuration and maintenance, diagnosis and treatment capabilities, cognition and preferences, regulations, systems and mechanisms. It also proposes to explore sustainable and diversified funding and guarantee channels in poverty-stricken areas, strengthen the construction of facilities, technologies and talents, promote the reform of the supply side of telemedicine services, stimulate the preference of doctors and patients, establish and improve relevant regulations, systems and mechanisms, etc. In order to promote the long-term development of health and poverty alleviation through telemedicine in the “post-poverty alleviation period” and the soundness of the long-term mechanism of health and poverty alleviation.

Novelty search consulting service for medical and health science and technology projects under the new situation

Under the new situation, doing a good job in medical and health science and technology novelty search and consultation service can promote the development of medical and health science and technology innovation, so it is closely related to people’s health level. This article first elaborates on the urgent need to do well in medical and health science and technology novelty search consulting services under the new situation, including that the level of medical and health science and technology innovation has been paid attention to by the country, which poses challenges to novelty search consulting services, and under the background of the coordinated development of Beijing-Tianjin-Hebei, the establishment of Xiong’an New Area in Hebei Province posed challenges for novelty search consulting services, and medical and health technology novelty search consulting services were widely recognized. Subsequently, the characteristics of medical and health science and technology novelty search consulting services are analyzed, including the wide range of business types of novelty search consulting services, the expansion of the data resources retrieved from periodicals to books, the subject contains many innovation points and the higher requirements for the compound quality of novelty search personnel. In view of the above characteristics, we have carried out exploratory work from three aspects: the establishment of a number of novelty retrieval and consultation WeChat groups, the completion of novelty retrieval by relevant professional novelty retrieval personnel, and the opening up of a “green channel” for novelty retrieval and consulting services from Xiong’an New Area in Hebei Province.

Impact of the “three-level linkage” elderly health management model on elderly health

Objective To explore the effect of carrying out “three-level linkage” health management model for the elderly. Methods Using the convenience sampling method, 335 elderly inpatients in Peking Union Medical College Hospital, Chinese Academy of Medical Sciences from February to August 2018 were selected as the research objects, and a one-year elderly health management was implemented, including health risk assessment, health risk monitoring, and health improvement. The quality of life, body composition, family function and social support of elderly patients before and after intervention were compared. Results One year after the implementation of the elderly health management model, the quality of life in addition to physiological functions, the protein, bone mineral content, skeletal muscle of body composition, family function scale and social support scale scores of elderly patients were all higher than before the intervention (P < 0.05), body weight and body fat in body composition were all lower than before intervention (P < 0.05). Conclusion The “three-level linkage” elderly health management model can effectively improve the quality of life of the elderly, improve the physiological indicators of the body, and at the same time improve the family function and social support of the elderly patients, thereby improving the health of the elderly patients.

Discussing the efficacy of rolling tuina in treating knee osteoarthritis based on AnyBody simulation technology

Objective To compare the changes and efficacy of the muscle force of the main muscle group of the lower limbs before and after the treatment of knee osteoarthritis patients by rolling Tuina. Methods The 15 KOA patients who were treated in the Department of Tuina, Yueyang Hospital of Integrated Traditional Chinese and Western Medicine, Shanghai University of Traditional Chinese Medicine, from January to June 2018 were selected and treated with rolling Tuina. The muscle force of the main muscle group of the lower limbs was analyzed by AnyBody simulation technology to compare the changes in muscle imbalance. Results After treatment with rolling Tuina, 15 KOA patients had achieved treatment effect, the degree of muscle imbalance of the anterior, posterior and lateral muscle group of the lower limbs had a tendency to decrease, among which the degree of muscle imbalances of rectus femoris, biceps femoris longhead and peroneal longus were significantly smaller than those before treatment, the differences were statistically significant (all P < 0.05). After treatment, the degree of muscle imbalance of medial muscle group showed an increasing trend, among which the degree of muscle imbalances of sartorius magnus were larger than that before treatment, but the degree of muscle imbalances of adductor magnus were smaller than that before treatment and the differences were statistically significant (all P < 0.05). Conclusion Rolling Tuina can improve the imbalance of lower limbs muscle in KOA patients. Its effective mechanism may be related to improvement the muscle force of the lower limbs muscles of KOA patients, especially the muscle force of the anterior and posterior muscles of the lower limbs, which can restore the coordination of flexor and extensor muscles. It is suggested that Tuina therapy for KOA should not only correct the osseous structural disorder, but also restore the muscle imbalance of lower limbs.

Effect of orthodontic-periodontal combination therapy on periodontal tissue of children with dislocation of teeth and secondary occlusal trauma

Objective To evaluate the effect of periodontal-orthodontic treatment on the local periodontal tissue of children with malposed teeth and secondary occlusal trauma in mixed dentition period. Methods From January 2016 to January 2018, 50 children in mixed dentition period were selected from Department of Stomatology, Xuzhou Central Hospital. They were divided into two groups according to the malposed teeth whether or not with secondary occlusal trauma, children in the study group with malposed teeth and secondary occlusal trauma (n = 25), children in the control group with malposed teeth without secondary occlusal trauma (n = 25). The periodontal status of all children were evaluated at the first visit before orthodontic-periodontal treatment. Probing pocket depth (PPD), gingival index (GI), bleeding index (BI) and plaque index (PI) of the two groups were compared before and after orthodontic-periodontal treatment. Results The average periodontal index PD, GI, BI and PI of malposed teeth with secondary occlusal trauman in the study group before treatment were higher than those in the control group, the differences were statistically significant (P < 0.05). After treatment, the local periodontal status of malposed teeth in the two groups was significantly improved compared with that before treatment, the differences were statistically significant (P < 0.05). But there was no significant difference in PPD, GI, BI and PI between two groups after treatment (P > 0.05). Conclusion Early intervention should be given to the mixed dentition children with malposed teeth and a secondary occlusal trauma. A combined orthodontic-periodontal therapy can not only eliminate occlusal trauma and align teeth, but also help to reconstruct and repair periodontal tissue.

Effect of Coicis Semen oil on intestinal flora and inflammatory factors in mice

Objective To investigate the effect of Coicis Semen oil on intestinal flora and inflammatory factors. Methods According to the random number table method, 75 SPF male Kunming mice were divided into blank control group, model group, low, medium and high dose group, with 15 mice in each group. Lincomycin was gavaged interfered with the intestinal flora of mice for modeling. After modeling, the model group was given 0.33 g/(kg·d) normal saline, while the low, medium and high dose group were given 0.17, 0.33 and 1.00 g/(kg·d) Coicis Semen oil for 30 days by gavage. The difference of colonic histopathological morphology in each group were observed. The number of Enterococcus, Enterobacter, Bifidobacterium and Lactobacillus, and the weight of mice in each group were compared. The levels of interleukin-6 (IL-6), C-C chemokine ligand 2 (CCL2), IL-1α and IL-1β were compared. Results In the blank control group, the pathological morphology of the colon was normal. In the model group, the inflammation of the pathological morphology was serious. In the low, medium and high dose groups, the pathological morphology was gradually improved compared with the model group. The number of Enterococcus and Enterobacter in the colon of mice in model group were higher than those in blank control group, the number of Bifidobacterium and Lactobacillus in model group were lower than those of the blank control group, and the differences were statistically significant (all P < 0.05). The number of Enterococcus and Enterobacter in the colon of mice in low, medium and high dose groups were lower than those in model group, the number of Bifidobacterium and Lactobacillus in low, medium and high dose groups were higher than those in model group, and the differences were statistically significant (all P < 0.05). On the 30th day, the weight of mice in the low, medium and high dose groups were higher than those in model group, and the differences were statistically significant (all P < 0.05). The levels of IL-6, CCL2, IL-1α and IL-1β in model group were higher than those in the blank control group, while the levels of IL-6, CCL2, IL-1α and IL-1β in the low, medium and high dose groups were lower than those in model group, and the differences were statistically significant (all P < 0.05). Conclusion Coicis Semen oil can improve the imbalance of intestinal flora and inhibit the expression of intestinal inflammatory factors in mice.

Effect of RNAⅢ inhibitor peptide on the expression of virulence factors of Staphylococcus aureus at different growth phases

Objective To explore the role of RNAⅢ inhibitory peptide (RIP) in regulating the expression of staphylolysin and adhesion-related proteins at different growth phases. Methods The methicillin-resistant Staphylococcus aureus was inoculated to medium proportionally, and the bacteria cultured overnight were transferred to TSB medium (containing 0.5% glucose), respectively, and the concentration was adjusted to 1×108 CFU/mL. The RIP group was added with RIP derivatives (RIP1183) designed and synthesized by research group, while the control group was added with sterile phosphate buffer. Its effects on the growth curve and biofilm formation were examined. Quantitative real-time fluorescence polymerase chain reaction was used to detect the expression levels of RNAⅢ, human leucocyte antigen (HLA), icaA, and fnbA during the bacterial retardation phase (2 h), logarithmic phase (6 h), and stable phase (10 h), and compared with the control group. Results Compared with the control group, RIP1183 did not affect the growth of bacteria, and the difference was not statistically significant (P > 0.05). Compared with the control group, the formation of bacterial biofilms in the RIP group was significantly reduced (P < 0.01). In the bacterial retardation phase (2 h), the expression of RNAⅢ in RIP group was obviously decreased (P < 0.05). In the logarithmic phase (6 h), compared with the control group, the expression of RNAⅢ and HLA were obviously decreased (P < 0.01). In the stable phase (10 h), the expression of icaA, fnbA and RNAⅢ were obviously decreased (P < 0.05 or P < 0.01). Conclusion RIP1183 inhibits the expression of RNAⅢ, an effector molecule of the accessory gene regulation system, and significantly reduces the expression of different virulence factors in different phases, thereby destroying the mechanism of bacteria’s adaptation to the environment, which may be one of the mechanisms of its antibacterial activity and anti-biofilm formation.

Preparation of Nifedipine Sustained-release Tablets (Ⅱ) technology and its influence on release rate

Objective To study the different prescription factors affecting the dissolution of Nifedipine Sustained-release Tablets(Ⅱ). Methods With the dissolution curves as indexes, the important factors affecting the release rate of Nifedipine Sustained-release Tablets (Ⅱ), such as the particle size of raw materials, fillers, adhesives, surfactants, film coating and so on, were systematically assessed. Results The key formulation parameters controlling the release rate of Nifedipine Sustained-release Tablets(Ⅱ) were established. The results showed that the particle size of raw material D90 was 10-40 μm, the proportion of lactose and microcrystalline cellulose was the same, the concentration of starch slurry was 10%, the dosage of Tween 80 was 0.5%, and the coating weight gain was 3%. Conclusion These established formulation parameters are reasonable and suitable for guiding the industrial production of Nifedipine Sustained-release Tablets (Ⅱ).

Establishment and validation of nomogram model for prediction of peripherally inserted central venous catheter-related bloodstream infection risk

Objective To discuss the risk factors of peripherally inserted central catheter (PICC) related bloodstream infection (PBSI) in patients with PICC, and to establish and validate a nomogram model for predicting the risk of PBSI. Methods A total of 931 patients receiving PICC in the Second Affiliated Hospital of Hainan Medical University from January 2016 to January 2020 were selected as research objects. The patients were divided into PBSI group (63 cases) and non-PBSI group (868 cases) according to whether there was PBSI or not, and clinical characteristics were compared. Multiplicity logistic regression analysis was used to screen independent risk factors of PBSI. According to the results of regression analysis, the nomogram prediction model was established. Calibration curve and receiver operating characteristic curve (ROC) were used to evaluate the predictive effectiveness of the model. Results Multiplicity logistic regression analysis showed that diabetes mellitus, malignant tumor, hematopathy, parenteral nutrition, double lumen, additional devices, ICU stay, and the time of indwelling catheter were independent risk factors for PBSI in PICC patients (P < 0.05). The C-index of internal validation of nomogram model was 0.929. The average absolute error between the predicted risk of PBSI and the actual risk was 0.017. The area under the curve predicted by ROC curve nomogram model was 0.930. Conclusion The nomogram model established in this study for predicting the risk of PBSI in patients with PICC has good discrimination and accuracy, and has high clinical value.

Current situation and prospect on the treatment of chemotherapy-induced alopecia in breast cancer patients based on traditional Chinese medicine

Chemotherapy-induced alopecia(CIA), as one of the frightening side effects for most breast cancer patients, has a negative effect on patients’ psychology state and compliance of treatment. Traditional Chinese medicine has unique advantages in the prevention and treatment of CIA with the theory that hair is the surplus of the blood, kidney stores the essence and has its manifestations in hair. The etiology and pathogenesis of CIA in traditional Chinese medicine are that the medicine pathogen of chemotherapeutics can cause the deficiency of both qi and blood as well as the deficiency of liver-kidney yin, thus lead to the undernutrition of hair or the stagnation of liver qi can then cause the combination of qi stagnation and blood stasis syndrome, which makes the stagnated blood lock collaterals, and the hair may also lose nourishment. So the treatment of CIA should be applied by invigorating qi and nourishing blood, tonifying liver and kidney, or promoting blood circulation to remove meridian obstruction respectively based on the traditional Chinese medicine syndrome differentiation and treatment system. Basic medical research showed that the mechanism of Chinese herbs in treating CIA is related to the inhibition of corticotropin-releasing factor receptor and prostaglandin D2 synthetase.

Hereditary ataxia (HA) is a class of hereditary neurodegenerative diseases with high clinical and genetic heterogeneity, mortality and disability rates. The disease has a high incidence in all groups of people in the world, with severe clinical symptoms and poor prognosis, seriously affecting the quality of life of patients. Western medicine mostly uses symptomatic treatment such as nourishing nerve, relieving spasm and anti-epilepsy to alleviate symptoms and delay progress, but the treatment effect is poor. Acupuncture and moxibustion treatment of this disease has achieved a good treatment effect, has a certain advantage. This paper summarizes and analyzes the current situation of traditional Chinese acupuncture and moxibustion in treating HA, in order to understand the latest progress of traditional Chinese acupuncture and moxibustion in treating HA, such as simple acupuncture, acupuncture combined with medicine and moxibustion, bloodletting and cupping, rehabilitation training, transcranial repeated magnetic stimulation and Western medicine, and to provide ideas for TCM diagnosis and treatment of this disease and further scientific research.

Research progress on the protective mechanism of Propofol on ischemic brain injury

Ischemic brain injury (IBI) is one of the main causes of adult disability. IBI causes neuro-inflammation, oxidative stress and cell apoptosis to damage neurons, and current treatments for IBI are limited. As a commonly used intravenous anesthetic in clinical practice, in addition to its sedative and hypnotic effects, Propofol also has the effects of reducing cerebral blood flow and lowering brain metabolism. It can be passed through phosphoinositide 3-kinase (PI3K)/serine-threonine protein kinase (Akt) pathway and Toll-like receptor 4 (TLR4)/nuclear factor (NF)-κB pathway play an anti-inflammatory effect, reduce the production of oxygen free radicals and resist cell apoptosis, and play a protective role in ischemic brain tissue and neurons. The protective effect of Propofol may become a new target for clinical treatment of IBI.

Analysis on the structure and coherence of drug use in public medical institutions in Qingdao

Objective Take Qingdao as an example to discuss the drug use structure of public medical institutions and the connection of drug use in lower-level medical institutions, and to provide reference for the promotion of graded diagnosis and treatment. Methods The stratified sampling method was used to select 13 public medical institutions in Qingdao, and the anatomical-treatment-chemical system (ATC) drug classification method was used to analyze the types and structures of drugs used by medical institutions at all levels from January 1 to December 31, 2017. Results The number of medicines in tertiary medical institutions was 1.73 times that of primary medical institutions and 1.59 times that of secondary medical institutions. There was a big difference in the commonality of medicines between primary and secondary and tertiary medical institutions, with the same medicine rates being 42.55% and 33.34% respectively. The allocation rates of essential medicines in first, second and third-level medical institutions were 60.88%, 31.83%, and 19.30% respectively. Conclusion The types of medicines in the first-level medical institutions are narrow, and the level of medication is low. There are obstacles to the connection with the second and third-level medical institutions. It is difficult to meet the medication needs under the hierarchical diagnosis and treatment model. The provision rate of basic drugs needs to be improved.

Survey of knowledge, attitude and practice of diet pagoda among junior preventive medicine undergraduates

Objective To understand the current status of knowledge, attitude and practice (KAP) of Chinese balance dietary pagoda of the lower grade preventive medicine undergraduates, and to provide a basis for school nutrition education and teaching reform. Methods Taking the 2018 and 2019 preventive medicine undergraduates of Shanxi Medical University as the research object, the self-compiled questionnaire was used to conduct an online survey in May 2020. The basic information and data on diet pagoda KAP were collected and statistical analysis were conducted. Results The total score of pagoda KAP was (31.32±6.47) points, knowledge score (5.14±2.26) points, attitude score (12.14±2.60) points, practice score (13.77±5.26) points. There was a statistically significant difference in the total KAP scores of students from different places of origin and monthly diet expenditure (P < 0.05). The attitude scores of girls were higher than boys, and the difference was statistically significant (P < 0.05). There was a statistically significant difference in the pagoda behavior scores of students with different genders, birthplaces, and monthly diet expenditures (P < 0.05). Among them, the scores of students with monthly dietary expenses <500 yuan were compared with those with 500-1000 yuan and >1000 yuan, and the differences were statistically significant (P < 0.05). Conclusion Lower grades of preventive medicine undergraduates have poor overall dietary knowledge, beliefs and behaviors. Although their attitudes are good, they have bad pagoda-related behaviors and lack pagoda knowledge. Nutrition publicity and education after school should be increased, and the teaching methods of nutrition education should be explored reforms.

Construction of the indicator system of sexual and reproductive health literacy of immigrated adolescents based on Delphi method

Objective To construct the evaluation index system of sexual and reproductive health literacy of immigrated adolescents, and to provide content basis and quantitative standards for scientific evaluation. Methods The research team used the literature research method and brainstorming method to construct the evaluation system consultation form. Twenty experts from nine tertiary general hospitals or women’s and children’s specialist hospitals in Beijing, Guangdong, Xinjiang, Sichuan, and Guizhou were selected to conduct two rounds of consultation and argumentation using Delphi expert consultation method. And the index weight was determined through the optimal sequence diagram method. Results The return rate of the two rounds of consultation questionnaires was 100.00%, and the expert authority coefficients were 0.81-0.89; the Kendall’s W coordination test expert opinions were consistent (P < 0.01). Through the first round of expert consultation: eight second-level indicators were deleted, and two second-level indicators were added; the second round of expert consultation: textual revisions were made to three second-level indicators. In the end, a system for evaluating sexual and reproductive health literacy of mobile adolescents including six primary indicators and 60 secondary indicators were obtained. Conclusion This questionnaire has a high degree of enthusiasm, authority and coordination of expert opinions. Status assessment of preconception health risk exposure among 11 408 males in different areas of Jiangsu Province

Objective To explore the pre-pregnancy health risk exposure of men in three regions of Jiangsu Province, and to analyze the pre-pregnancy health risk exposure of men. Methods Using the “core indicators of pre-pregnancy health risk exposure assessment for couples of childbearing age in China”, men in three project cities/counties in southern Jiangsu, central Jiangsu, and northern Jiangsu who participated in free pre-pregnancy eugenic health check in 2015 were used to conduct pre-pregnancy risk exposure assessment and their pre-pregnancy risk were analyzed. Results A total of 11 408 men were included in the study. There was a statistically significant difference in age, ethnicity, education level, occupation, and nature of household registration among people in different regions (P < 0.05). There was a statistically significant difference in the high-risk exposure status of behavioral risk, genetic risk, infectious risk and other risks among people in different regions (P < 0.05). There was a statistically significant difference in the high-risk exposure status of body mass index, blood pressure, heart rate, glutamic-pyruvic transaminase and creatinine among people in different regions (P <0.05). A total of 614 men were assessed as high-risk groups, with a high-risk rate of 5.38%. The top three behavioral risks with the highest risk rate were passive smoking, drinking, and smoking, which were 21.33%, 16.64% and 15.66%, respectively. Conclusion Men in Jiangsu Province are in good health before pregnancy, but there are certain behavioral risk exposures. In order to give birth to healthy children, pre-pregnancy eugenic health checkups are required. Prevent birth defects from the source and promote prenatal and postnatal care.

Investigation on smoke exposure in operating room and countermeasure analysis of prevention and management

Objective To investigate the smoke exposure in the operating room and to discuss the corresponding prevention and management measures. Methods A total of 68 nurses in the operating room of the Affiliated Hospital of Southwest Medical University were selected as the smoke exposure group. A general questionnaire was used to investigate the smoke exposure in the operating room. A total of 50 nurses in the non-operating room during the same period were selected as the non-smoke exposure group. The results of blood lipids, blood routine and renal function of the two groups were compared and analyzed, and corresponding preventive management measures were taken for the smoke exposure group. Results Compared with the non-smoke exposure group, the composition ratio of nurses’ perception of surgical smoke in the smoke exposure group was statistically significant (P < 0.05). The total score of nurses in smoke exposure group were higher than those in non-smoke exposure group, and the differences were highly statistically significant (P < 0.01). The total score for the implementation of surgical smoke protection was (5.43±1.67) points, and most nurses did not take effective protective measures. Serum total cholesterol, low-density lipoprotein, hematocrit, hemoglobin and uric acid of nurses in the smoke exposure group were significantly higher than those in the non-smoke exposure group, and the differences were statistically significant (P < 0.05). Conclusion The smoke in the operating room has a huge impact on the health of nurses, but the relevant cognition level and protection awareness of the practitioners are poor, and corresponding preventive management should be done to ensure their health.

Effect of Danggui Buxue Decoction and Ermiao Powder addition and subtraction combined with vacuum sealing drainage technique on diabetic foot

Objective To explore the effect of Danggui Buxue Decoction combined with Ermiao Powder addition and subtraction combined with vacuum sealing drainage (VSD) technique on diabetic foot (DF). Methods From September 2017 to October 2019, 80 patients with DF from Qinhuangdao Military Hospital in Hebei Province were selected as the research subjects, the patients were divided into observation group and control group by random number table method, 40 cases in each group. Both groups were given basic treatment + debridement and dressing change. Based on this, the control group was given VSD technology, and the observation group was given Danggui Buxue Decoction and Ermiao Powder addition and subtraction combined with VSD technology. The clinical efficacy, symptom improvement (skin temperature, foot pain, bipedal ankle brachial index [ABI]), inflammatory indicators (procalcitonin [PCT], C-reactive protein [CRP]) and serum insulin-like growth factor-1 (IGF-1), pigment epithelium-derived factor (PEDF) levels were compared between the two groups before and after treatment. Results The total effective rate of the observation group was higher than that of the control group (P < 0.05); after treatment, the skin temperature of the two groups was higher than before, and the pain score of the affected foot was lower than before (P < 0.05), the skin temperature and ABI index of the observation group after treatment were higher than the control group, and the pain score of the affected foot was lower than the control group (P < 0.05); after treatment, the levels of CRP, PCT and PEDF in both groups were lower than before treatment, and the levels of IGF-1 were higher than before treatment (P < 0.05); after treatment, the levels of CRP, PCT and PEDF in the observation group were lower than those in the control group, and the levels of IGF-1 were higher than those in the control group (P < 0.05). Conclusion Danggui Buxue Decoction and Ermiao Powder addition and subtraction combined with VSD technology for the first time in the treatment of DF, the effect is definite, can effectively improve clinical symptoms, shorten wound healing time, reduce inflammatory levels, and adjust serum levels of PEDF and IGF-1.

Application effect of mobile terminal APP in the health management of type 2 diabetes mellitus

Objective To evaluate the application effect of mobile terminal APP in the health management of type 2 diabetes mellitus (T2DM). Methods A total of 100 T2DM patients admitted to the First Hospital of Handan City, Hebei Province from January to December 2018 were selected as the study subjects. According to the random number method, the patients were divided into control group and observation group, 50 patients, in each group. The control group implemented traditional self-health management, while the observation group downloaded and installed the “Diabetes management” APP with smart phones and connected it with the doctor’s terminal to implement health management. After 12 months of follow-up, fasting blood glucose (FPG), 2-h postprandial blood glucose (2hPG), hemoglobin A1c (HbA1c), incidence of hypoglycemia, rehospitalization rate and diabetes-related complications were compared between the two groups. Results Overall analysis showed that there were statistically significant differences in intergroup comparison, time point comparison and interaction (P < 0.05). The differences of FPG, 2hPG and HbA1c between the two groups at different time points were statistically significant (P < 0.05). Further pairwise comparison and intra-group comparison: 6 and 12 months after intervention, FPG, 2hPG and HbA1c levels in both groups were significantly lower than those before intervention (P < 0.05). At 6 and 12 months after intervention, FPG, 2hPG and HbA1c levels in the observation group were lower than those in the control group, with statistically significant differences (P < 0.05). The incidence of hypoglycemia, rehospitalization rate and total incidence of diabetes-related complications in the observation group were all lower than those in the control group, with statistically significant differences (P < 0.05). Conclusion The application effect of mobile terminal APP in T2DM health management is significant, which can significantly reduce complications, improve blood glucose level and prognosis.

Effect of Mirabegron on ureteral stent-related symptoms

Objective To study the effect of Mirabegron on ureteral stent-related symptoms (SRSs). Methods From January to December 2019, 120 cases with indwelling ureteric stents after surgery for upper urinary tract calculi were selected as research subjects. They were divided into the study group and the control group according to random number table method, with 60 cases in each group. The study group was treated with Mirabegron, while the control group did not use any drugs for lower urinary tract symptoms. At the time of stent removal, the international prostate symptom score (IPSS), quality of life (QoL) score, patient perception of bladder condition (PPBC) score, visual analog scale (VAS) score and the amount of analgesics were compared between the two groups. Results Excluded the cases that lost to follow-up or received other treatment options, 102 cases completed the study: 50 cases in the study group, 52 cases in the control group. At the time of stent removal, the scores of IPSS (storage symptoms), QoL, PPBC, VAS and the amount of analgesics in the study group were lower than those in the control group, and the differences were statisticaliy significant (P < 0.05); there was no significant difference in the score of IPSS (voiding symptoms) between the two groups (P > 0.05). Conclusion Mirabegron has obvious effect on the treatment of SRSs, which can reduce the symptoms of bladder irritation and the amount of analgesic usage.

Advantages of 3D printing simulation surgery method in the proximal humeral Neer four-part fractures and its influence on postoperative functional recovery

Objective To explore the advantages of 3D printing simulation surgery method in the proximal humeral Neer four-part fractures and the effect of postoperative function recovery. Methods A total of 60 patients with proximal humeral Neer four-part fractures who were admitted to Liyang Branch, Jiangsu Provincial People’s Hospital from January 2017 to January 2020 were selected and divided into 3D group and traditional group by random number table method, with 30 patients in each group. Both groups were treated with locking plate combined with minimally invasive plate osteosynthesis, and the 3D group was treated with three-dimensional reconstruction to establish 3D printing model. After simulating the implementation of surgical engineering, the patients were operated according to the plan. Operative time, amount of surgical bleeding, number of intraoperative C-arm fluoroscopy, anatomic reduction rate, fracture healing time, shoulder joint function of patients at postoperative difference time (Constant-Murley score) and surgical complications were compared between the two groups. Results Operative time, amount of surgical bleeding, number of intraoperative C-arm fluoroscopy and fracture healing time in the 3D group were all lower than those in the traditional group, with statistically significant differences (all P < 0.05). The anatomic reduction rate of the 3D group was higher than that of the traditional group, and the difference was statistically significant (P < 0.05). Six months after the surgery, pain, muscle strength, daily activity, range of joint activity and Constant-Murley total score were all increased in the two groups compared with the three months after the surgery, and the differences were statistically significant (all P < 0.05). Three months after the surgery, pain, muscle strength, daily activity, range of joint activity and Constant-Murley total score in the 3D group were all higher than those in the traditional group, with statistically significant differences (all P < 0.05). Six months after the surgery, pain, muscle strength, daily activity, range of joint activity and Constant-Murley total score in the 3D group were not significantly different from those in the traditional group (all P > 0.05). The incidence rate of surgical complications of the 3D group was lower than that of the traditional group, and the difference was statistically significant (P < 0.05). Conclusion The application of 3D printing simulation surgery method in the proximal humeral Neer four-part fractures has certain advantages in shortening the operation time, reducing the surgical bleeding, improving the reduction effect, and improving the early recovery of shoulder joint function after the surgery.

Effect of shock wave combined with electroacupuncture on residual skin sensation disorder in lumbar 5 innervation area after spinal endoscopy

Objective To analyze the clinical effect of shock wave combined with electroacupuncture on residual skin sensation disorder in lumbar 5 innervation area after spinal endoscopy. Methods From January 2018 to June 2019, seventy-five patients were admitted to the First Affiliated Hospital of Hebei University of Chinese Medicine and the Hospital of medical association who were diagnosed as L4/5 disc herniation and residual skin sensation disorder in lumbar 5 nerve innervation area after percutaneous endoscopic lumbar discectomy were included. According to the random number table method, they were divided into electroacupuncture group, shock wave group and combined group, with 25 cases in each group. The electroacupuncture group received electroacupuncture treatment, the shock wave group received extracorporeal shock wave treatment, and the combined group received extracorporeal shock wave combined electroacupuncture treatment. The treatment took two weeks as one course of treatment, a total of two courses of treatment. Visual analogue scales (VAS) of numbness scores and Japanese orthopaedic association (JOA) lumbar pain assessment standard scores were recorded before treatment, after one course of treatment, and after two courses of treatment. The therapeutic effect and clinical efficacy were evaluated. Results Overall analysis showed that the differences among groups, time points and interactions were statistically significant (all P < 0.05). Compared with before treatment, VAS scores of numbness decreased and JOA scores increased after one or two courses of treatment in the three groups, with statistically significant differences (all P < 0.05). After treatment, VAS score of numbness in the combined group was lower than that in the electroacupuncture group and shock wave group, and JOA score was higher than that in the electroacupuncture group and shock wave group. There was no significant difference in VAS and JOA scores between the electroacupuncture group and the shock wave group (P > 0.05). The overall clinical efficacy of the combined group was higher than that of the electroacupuncture group and shock wave group (P < 0.017). Conclusion Shock wave combined with electroacupuncture has a good clinical effect on residual skin sensation disorder in the lumbar 5 nerve innervation area after percutaneous endoscopic lumbar discectomy, and its therapeutic effect is superior to other monotherapy regiments, and can significantly relieve the symptoms of skin sensation disorder after percutaneous endoscopic lumbar discectomy.

Effect of breast conservative surgery combined with sentinel lymph node biopsy on the armpit extubation time, shoulder range of motion and long-term outcomes of patients with early breast cancer

Objective To explore the effect of breast conservative surgery combined with sentinel lymph node biopsy on the armpit extubation time, shoulder range of motion and long-term outcomes of patients with early breast cancer. Methods The medical records of 92 cases of early breast cancer treated in the First Affiliated Hospital of Xi’an Jiaotong University from October 2015 to October 2017 were studied. Of which 44 cases received breast conservative surgery combined with sentinel lymph node biopsy (observation group) and 48 cases received modified radical mastectomy (control group). Perioperative parameters (length of operation, amount of bleeding during operation, total volume of drainage, armpit extubation time, length of stay), shoulder range of motion (outward-rotating, inward-rotating, backward extension, outreaching, bending) before and after surgery, postoperative complications, satisfaction with the result of breast cosmetology, scores of the specific questionnaire to patients with breast cancer after surgery (QLQ-BR23), and results of the follow-up survey of patients in two groups were compared. Results The operation time of observation group was longer than that of control group, the intraoperative blood loss and total drainage volume of observation group were less than those of control group, the axillary extubation time and hospitalization time of observation group were shorter than those of control group, the differences were statistically significant (P < 0.05). Six months after operation, the external rotation, internal rotation, extension, abduction and flexion of shoulder joint in two groups were decreased compared with those before operation (P < 0.05), but observation group was larger than that of control group (P < 0.05). The incidence of complication in observation group was lower than that in control group (P < 0.05). The satisfaction of breast beauty in observation group was higher than that in control group six months after operation (P < 0.05). The reliability and validity of the self-made questionnaire of breast beauty satisfaction: Cronbach’s α coefficient was 0.854, test-retest reliability was 0.836. The content validity index was 0.817. The scores of body image, sexual function, sexual pleasure and future view in observation group were higher than those in control group six months after operation, while the scores of breast symptoms and upper limb symptoms in observation group were lower than those in control group (P < 0.05). There were no significant differences in the scores of hair loss and side effects between two groups (P > 0.05). Followed up for one year, there were no significant differences in survival rate, recurrence rate and metastasis rate between two groups (P > 0.05). Conclusion The treatment of breast conservative surgery combined with sentinel lymph node biopsy can produce a therapeutic effect close to that of modified radical mastectomy in patients with early breast cancer, while having such advantages as faster recovery after operation and smaller impact on the appearance and activities of daily living of the breasts.

Effect of improvement of standard use of magnesium sulfate in the pregnancy outcomes of patients with severe pre-eclampsia

Objective To evaluate the pharmacy monitoring outcomes of patients with severe pre-eclampsia by promoting the standard use of magnesium sulfate after pharmaceutical care. Methods From July 2016 to June 2017, the medical records of patients with severe pre-eclampsia admitted to Women’s Hospital of Nanjing Medical University were collected and as control group (178 cases). Pharmaceutical care plan was formulated from July to December 2017. Patients with severe pre-eclampsia admitted from January to December 2018 were selected as intervention group (185 cases). The results of pharmaceutical intervention were recorded. The use of magnesium sulfate before pregnancy, after pregnancy and within 32 weeks of pregnancy, the treatment schedule and pregnancy outcomes were compared between the two groups. Results The number of overall utilization of magnesium sulfate, the utilization of prenatal magnesium sulfate, the utilization of postpartum magnesium sulfate, the utilization of both prenatal and postpartum magnesium sulfate in intervention group were larger than those in control group (P < 0.05). There were no significant differences in the daily dose and duration of magnesium sulfate maintenance, postpartum magnesium sulfate administration time, total prenatal and postpartum medication time, and the number and level of magnesium ion monitoring between two groups (P > 0.05). The number of utilization of magnesium sulfate in intervention group was larger than that in control group (P < 0.05). There was no significant difference in the incidence of utilization of magnesium sulfate between two groups (P > 0.05). There were no significant differences in the eclampsia, mode of delivery, time of termination of pregnancy and average weight of all fetuses between two groups (P > 0.05). The duration of pregnancy in intervention group was longer than that in control group (P < 0.05). Conclusion Magnesium sulfate in preventing eclampsia in patients with severe pre-eclampsia is still lack of clinical use. By implementing pharmaceutical care measures, the standard use of magnesium sulfate can be promoted and also improve the pregnancy outcomes.

Study on the value of gene chip technology in the case of thickening of the neck transparent layer

Objective To explore clinical application value of chromosome microarray analysis (CMA) in the thickening of nuchal translucency (NT) and the difference of detection of copy number variants (CNVs). Methods A retrospective analysis was conducted from December 2017 to December 2019. Ultrasound prompted “thickening of the transparent layer of the fetal neck”. A karyotype analysis and CMA chip detection of 185 pregnant women with singletons were performed at Maternal and Child Health Hospital of Inner Mongolia Autonomous Region. Results CMA test results: a total of 185 cases of NT thickening, 59 cases with chromosomal abnormalities, the incidence rate of chromosomal abnormalities was 31.89%(59/185). Compared with karyotype analysis, the detection rate of CNVs increased by 10.81%(20/185), and the detection rate of pathogenic CNVs increased by 7.03%(13/185). Chromosome karyotype analysis results: a total of 185 cases of NT thickening, 39 cases with abnormal karyotypes, the incidence rate of chromosomal abnormalities was 21.08%(39/185). There were two cases with abnormal chromosome structure: one case of 46, XN, del (18) (P11) and 18p11.32p11.2 deletion in CMA was 14.94 Mb, P was the same case. One case of 46, XN, t (15; 17) (Q15; q25) [4]/46, XN [122], CMA was 46, XN. Conclusion Compared with karyotype analysis technology, CMA technology has the advantage of high genome-wide resolution of CMA technology. It can increase the detection rate of microdeletions/microduplications and the detection rate of pathogenic CNVs. Unable to recognize balanced rearrangements (translocation/inversion/insertion), CMA technology can not replace karyotype analysis technology for diagnosis. The existence of unclear clinical significance (VOUS) of CMA detection will bring difficulties to the interpretation of CMA.

Clinical effect of topical Nocardia rubra cell wall skeleton in the treatment of persistent high-risk HPV infection of the cervix

Objective To explore clinical effect of topical Nocardia rubra cell wall skeletal (Nr-CWS) in the treatment of cervical high-risk human papillomavirus (HPV) infection. Methods All of 273 cervical high-risk HPV patients admitted to Affiliated Hospital of Xuzhou Medical University from June 2018 to January 2020 were collected, they were divided into observation group (n = 136) and control group (n = 137). Observation group was treated with Nr-CWS, while control group was treated with recombinant human interferon α-2b gel. Clinical efficacy after treatment of three months and six months and adverse reactions during treatment were compared between two groups. Results Total effective rate of observation group after treatment of three months and six months was significantly higher than that of control group (P < 0.05). There was no significant difference in total effective rate between single subtype infection subgroup and multiple subtype infection subgroup (P > 0.05). Total effective rate of HPV-16 subgroup, HPV-16 with other subgroups, HPV-18 with other subgroups, HPV-18 with HPV-16 subgroups, HPV-18 with HPV-16 subgroups, single other high-risk subtype subgroups and multiple other high-risk subtype subgroups were higher than those of HPV-18 subgroup (P < 0.05). Total effective rates of HPV-16 subgroup, HPV-16 with other subgroups, HPV-18 with other subgroups, HPV-18 with HPV-16 subgroups and HPV-18 with HPV-16 and other subgroups were higher than those of single other high-risk subtype subgroups and multiple other high-risk subgroups (P < 0.05). There was no significant difference in the incidence rate of adverse reaction between two groups during treatment (P > 0.05). Conclusion The efficacy of topical Nr-CWS in the treatment of cervical high-risk HPV infection is better than that of recombinant human interferon α-2b gel, which is worthy of clinical promotion.

Comparative empirical study on two methods of disease cost accounting

Objective To compare and analyze two types of cost accounting methods. Method Two cost account methods (Method 1 was based on patients’ payment and Method 2 was cost accounting of disease species based on clinical path cost locationon) was used to measure the cost of hip osteoarthritis (primary or secondary), total hip arthroplasty; severe knee osteoarthritis, total knee arthroplasty; chronic tonsillitis, tonsillectomy; senile cataract, cataract phacoemulsification + intraocular lens implantation; primary hepatocellular carcinoma, partial hepatectomy or hepatectomy among 5 sample hospitals in 2019. Results In Method 1, the difference in direct human cost leaded to great distinction in total cost and cost balance among sample hospitals. Besides, most direct cost accounted for about 80% using Method 1, while most of the direct costs in method 2 were less than 80% using Method 2. The medical material expense of some disease measured by Method 2 was lower than that measured by Method 1. Conclusion As for Method 1, it is relatively difficult to acquire required data, but it can meet the needs of hospital internal management. While as for Method 2, it is easy to obtain necessary data, and it is conducive to the standardize the cost path and analyze the causes of abnormal disease cost.

Determination on the contents of 6 active ingredients in Glycyrrhizae Radix et Rhizoma by solid phase extraction and high performance liquid chromatography

Objective To establish a quantitative method of solid phase extraction and high performance liquid chromatography in determination of six active ingredients in Glycyrrhizae Radix et Rhizoma from different habitats. Methods The effective constituent of Glycyrrhizae Radix et Rhizoma was extracted by solid phase extraction and its active ingredients were determined by high performance liquid chromatography (HPLC). The mobile phase was acetonitrile-0.05% phosphoric acid solution, with gradient elution, flow rate of 1 mL/min, and detection wavelength of 240 nm. Results The six active ingredients albiflorin, paeoniflorin, liquiritin apioside, liquiritin, benzoylpaeoniflorin, glycyrrhizic acid in Glycyrrhizae Radix et Rhizoma showed good linear relationship in the range of 28.8-920.0, 14.7-470.0, 61.9-1980.0, 48.4-1550.0, 71.9-2300.0, 27.5-880.0 μg/mL, the recovery rate was 98.44%, 101.25%, 96.78%, 103.74%, 97.85%, 95.87% respectively, and the RSD was 1.98%, 3.02%, 2.85%, 1.99%, 2.47%, 2.06% respectively. Conclusion Solid phase extraction combined with HPLC in detection of the active ingredients of Glycyrrhizae Radix et Rhizoma is fast and reliable.

Clinical effect of Alprostadil combined with Compound Biejia Ruangan Tablets and Entecavir in the treatment of HBeAg positive patients with decompensated liver cirrhosis

Objective To explore clinical effect of Alprostadil combined with Compound Biejia Ruangan Tablets and Entecavir in the treatment of hepatitis B e antigen (HBeAg) positive patients with decompensated liver cirrhosis. Methods One hundred and eight patients with HBeAg positive hepatitis B decompensated cirrhosis hospitalized in Tangshan Central Hospital from June 2018 to June 2019 were selected and divided into control group and case group according to the random number table method, with 54 cases in each group. Control group was given Compound Biejia Ruangan Tablets and Entecavir, and case group was given Alprostadil treatment based on control group. The course of treatment was four weeks. The liver and kidney function, serum inflammatory factors, virological indexes and clinical efficacy between two groups were observed before and after treatment. Results After treatment, the levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST) and alkaline phosphatase (ALP) in two groups were lower than those before treatment, and the level of albumin (ALB) was higher than that before treatment, and the levels of ALT, AST and ALP in case group were lower than those in control group, and ALB level was higher than that in control group (P < 0.05). After treatment, the levels of laminin, hyaluronic acid, type Ⅲ procollagen and type Ⅳ collagen in two groups were lower than those before treatment, and those in case group were lower than those in control group (P < 0.05). After treatment, the portal vein diameter (Dpv) and splenic vein diameter (Dsv) of two groups were shorter than those before treatment, and the portal vein blood flow velocity (Vpv), portal vein blood flow (Qpv), splenic vein blood flow velocity (Vsv), and splenic vein blood flow (Qsv) were faster than those before treatment, and Dpv and Dsv of case group were shorter than those of control group, and Vpv, Qpv, Vsv and Qsv of case group were faster than those of control group (P < 0.05). After treatment, blood urea nitrogen (BUN), creatinine (Cr) and resistance index of two groups were lower than those before treatment, and renal blood flow (RBF) was faster than that before treatment, and BUN, Cr and resistance index of case group were lower than those of control group, and RBF of case group was faster than that of control group (P < 0.05). After treatment, the serum levels of interleukin-6, interleukin-8, tumor necrosis factor-α and C-reactive protein in two groups were lower than those before treatment, and those in case group were lower than those in control group (P < 0.05). After treatment, the levels of fibrinogen like protein 2 (FGL2), toll like receptor 4 (TLR-4), transforming growth factor beta 1 (TGF-β1) and aspartate aminotransferase platelet ratio index (APRI) in two groups were lower than those before treatment, and those in case group were lower than those in control group (P < 0.05). After treatment, the negative rate of HBV DNA, HBsAg and HBeAg in case group were higher than those in control group (P < 0.05). After treatment, total effective rate of case group was higher than that of control group (P < 0.05). Conclusion Alprostadil combined with Compound Biejia Ruangan Tablets and Entecavir can improve liver and kidney function, hepatic fibrosis and portal hemodynamics in patients with HBeAg positive decompensated liver cirrhosis, reduce HBV DNA load and inflammation, reduce FGL2, TLR-4, TGF-β1 and APRI levels, improve the body’s immune status.

Exploration on the medication rules of the treatment of tremor syndrome in ancient literatures based on data mining method

Objective To find the rules of ancient documents of traditional Chinese medicine in the treatment of tremor syndrome with data mining method. Methods Based on the data of Chinese Medical Canon and Guangzhou University of Chinese Medicine and the library of Guangdong Provincial Hospital of Traditional Chinese Medicine, the database of diagnosis and treatment of tremor syndrome in ancient books was established. The ancient and modern medical case cloud platform V1.7 was used. Frequency statistics, clustering analysis, association rules, complex network and other data mining technologies were used for analysis. Results A total of 177 prescriptions were included, involving 264 kinds of drugs. Its main properties were warm, sweet, and mainly belong to liver, spleen, lung meridian. The common herb pairs included “Angelicae Sinensis Radix-Ginseng Radix et Rhizoma”, “Astragali Radix-Ginseng Radix et Rhizoma”, “Schisandrae Chinensis Fructus”, and so on. High frequency drugs included Poria, Ginseng Radix et Rhizoma, Glycyrrhizae Radix et Rhizoma Praeparata Cum Melle, Atractylodis Macrocephalae Rhizoma, Angelicae Sinensis Radix, and so on. Combined with cluster analysis and complex network analysis, the core drug combination can be considered as Bazhen Decoction, Tianma Gouteng Decoction and Dihuang Yinzi. Conclusion The treatment of tremor syndrome in ancient documents of traditional Chinese medicine usually start from treatments of liver and spleen and mainly focus on usages of warm drugs. The common function of prescription for tremor syndrome in ancient documents include the effects of invigorating spleen and benefiting qi, suppressing hyperactive liver for claming endogenous wind, relieving muscle stiffness and pain, nourishing blood and reinforcing deficiency.

Influence of suiyun master’s time on the incidence of breast lump based on the theory of yunqi

Breast lump is a common and frequent disease among women of childbearing period. Syndrome differentiation and treatment of diseases with yunqi theory embody the idea of “correspondence between human body and natural environment” in traditional Chinese medicine, highlight the impact of natural climate change on disease occurrence. From the point of view of the theory of yunqi, the occurrence of breast lump is mainly due to the excessive transport of year evolutive phases, the prevalence of benqi, or the lack of year evolutive plases and zangqi, which causes the normal physiological dysfunction of the liver, spleen, kidney, Chong and Conception channels, and other organs and channels. In the excessive year of muyun, tuyun and jinyun, the lack year of muyun, tuyun and jinyun, the effect of natural climate change on human body caused the most obvious changes of liver loss, spleen deficiency and dampness accumulation, and imbalance of Chong and Conception channels. Breast lump is relatively frequent. In addition, the qi of celestial manager qi and qi in the earth in some years and the “assimilation of yun and qi” of suiyun can further affect the occurrence of breast lump. Application of the theory of yunqi to explain the occurrence of breast lump, it enriches the etiology and pathogenesis of mastitis. It has played an important role in guiding clinical syndrome differentiation and treatment, health preservation and disease prevention.

Application of opening sweat pores in the treatment of ischemic white matter lesions

This paper aims to investigate the application of opening sweat pores in the treatment of ischemic white matter lesions. By introducing the theory of sweat pores, this paper analyzes that the dysfunction of the opening and closing of sweat pores is the pathogenesis characteristic of ischemic white matter lesions, puts forward that direct dredging or indirect dredging sweat pores should be used to treat the disease, and wind drugs, insect drugs and resuscitation with aromatic herbs are commonly used drugs. According to the different pathogenesis characteristics of the dysfunction of the opening and closing of sweat pores of ischemic white matter lesions, benefiting both qi and blood, regulating qi and resolving stagnation, dissipating phlegm and eliminating dampness, promoting blood circulation for removing blood stasis, clearing heat-fire, dredging collaterals and removing toxicity can open sweat pores. Opening sweat pores can provide new ideas for syndrome differentiation and treatment of ischemic white matter lesions.

Exploration on the medication rule of Professor Sun Shidao in the treatment of atopic dermatitis based on data mining

Objective To explore the medication experience of Professor Sun Shidao of Shanghai famous traditional Chinese medicine physician in the treatment of atopic dermatitis (AD). Methods The prescriptions treated with AD of Professor Sun Shidao in Special Outpatient Department of Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from January 2015 to January 2018 were collected. Based on the software of traditional Chinese medicine inheritance support system, unsupervised data mining methods such as association rule algorithm and entropy clustering of complex system were adopted. The prescription characteristics and medication rules of Professor Sun Shidao in the treatment of AD were analyzed. Results A total of 274 prescriptions were included in this study, involving 189 kinds of medicine. The drugs used more than 70 times included 18 kinds of medicine such as Cortex Moutan Radicis, Radix Sophorae Flavescentis, Rhizoma Anemarrhenae, Cortex Dictamni, Radix Scutellariae, Radix Scrophulariae, Rhizoma Smilacis Glabrae. The character of highest frequency used medicine mainly concentrated in cold, bitter, and liver channel of traditional Chinese medicine meridian. Seventeen drug combinations with a confidence level greater than 0.98 were obtained, eight core drug combinations were extracted, and four potential new therapeutic prescriptions were mined. Conclusion Professor Sun Shidao treats AD mainly by clearing heat, cooling blood and removing dampness, supplemented by invigorating spleen and benefiting qi, dispelling pathogenic wind and relieving itching.

Study on the traditional Chinese medicine treatment and prescription rules of postcholecystectomy syndrome

Objective To explore traditional Chinese medicine treatment and prescription rules of postcholecystectomy syndrome (PCS) by data mining. Methods CNKI, Wanfang Data, VIP and PubMed were searched by computer, with the theme of “postcholecystectomy syndrome”, and the retrieval time was from the date of database construction to December 31, 2019. The literatures with TCM treatment were selected for discrimination. After establishing the database, frequency analysis, cluster analysis and association rule analysis were carried out. Results TCM treatment included dispersing stagnated liver qi for promoting bile flow, dispersing liver and regulating qi, clearing heat and eliminating dampness, etc. The prescriptions mainly included Chaihu Shugan Powder, Da Chaihu Decoction, Shugan Lidan Decoction (self-made). Association rules suggested that there were six kinds of commonly used two-drug pairs with support >40% and three kinds of three-drug pairs. Conclusion PCS can be divided into six syndrome types: syndrome of qi stagnation of liver and gallbladder, syndrome of dampness-heat of liver and gallbladder, pattern of static blood obstruction and stagnation, syndrome of stagnation of liver qi and spleen deficiency, syndrome of incoordination between liver and stomach, and spleen-stomach weakness syndrome, which provide references for clinical treatment of TCM.

Clinical experience of Professor Hu Dongju in treating anemia of hematopathy with ointment formula

Anemia is one of the common clinical manifestations in hematopathy. Due to the protracted and refractory characteristics of hematopathy, the treatment of anemia usually lasts for a long time, leading to poor compliance and even psychological boredom of patients, thus affecting the treatment effect. In view of this phenomenon, Professor Hu Dongju proposes to use ointment formula to treat anemia of hematopathy, make full use of the advantages that the ointment formula is easy to carry and tastes good. Combining with the invigorating characteristics of ointment formula, the longer course of hematopathy, and the pathogenesis of more deficiency and more stasis of hematopathy, Professor Hu Dongju puts forward the viewpoint of “emphasizing the spleen and kidney alone, attacking and supplementing simultaneously, adjusting measures for three reasons”, which improves the degree of acceptance and treatment effect of patients.

Application of Rhei Radix et Rhizoma topical application at shénquē combined with gastrointestinal pacing in elderly patients with functional constipation

Objective To investigate the application effect of Rhei Radix et Rhizoma topical application at shénquē combined with gastrointestinal pacing in elderly patients with functional constipation (FC). Methods Sixty elderly patients with FC admitted to Nantong Hospital of Traditional Chinese Medicine from September 2018 to March 2020 were selected as the research objects, and they were divided into control group and experimental group by random number table method, with 30 cases in each group. The control group was treated with life guidance and gastrointestinal pacing, on basis of control group, the experimental group was treated with Rhei Radix et Rhizoma topical application at shénquē. After four courses (10 days of continuous treatment was taken as one course) of treatment, the efficacy of the two groups was evaluated; before treatment and after 4 courses of treatment, the improvement conditions of anal canal motor function (maximum squeeze pressure [MSP] of anal canal, duration sustained squeeze [DSS] and anal canal resting pressure [RP] level) was tested and compared. Results After treatment, the efficacy of the experimental group was better than that of the control group, the difference was statistically significant (P < 0.05). After treatment, the MSP and RP in both groups were lower than those before treatment, and the experimental group was lower than the control group; the DSS in both groups was shorter than that before treatment, and the experimental group was shorter than the control group, the differences were statistically significant (P < 0.05). Conclusion The Rhei Radix et Rhizoma topical application of shénquē combined with gastrointestinal pacing in the treatment of elderly patients with FC can improve the anal canal motor function, and the clinical effect is definite.

Clinical observation of needle knife combined with manipulation and muscle strength training in the treatment of chronic sacroiliac joint dysfunction

Objective To observe the clinical effect of needle knife combined with manipulation and muscle strength training in the treatment of chronic sacroiliac joint dysfunction. Methods Sixty patients diagnosed with chronic sacroiliac joint dysfunction in the Department of Spine Orthopaedics, Wuxi Hospital of Traditional Chinese Medicine, Jiangsu Province from January 2017 to December 2019 were selected and divided into observation group and control group according to the random number table method, with 30 patients in each group. The control group was treated with manipulation and muscle strength training, and the observation group was treated with needle knife therapy on this basis. Visual analogue scale (VAS), Oswestry disability index (ODI), ilium transverse diameter difference and clinical efficacy were compared between the two groups. Results Overall analysis showed that VAS, ODI and ilium transverse diameter differences were statistically significant among groups, time point comparison and interaction differences (all P < 0.05). Intra-group comparison: VAS, ODI and ilium transverse diametral differences between the two groups were all lower than those before treatment at 15 days and six weeks after treatment, and VAS and ODI at six weeks after treatment were lower those that at 15 days after treatment, with statistically significant differences (all P < 0.05). Comparison between groups: After 15 days and six weeks of treatment, VAS, ODI and ilium transverse diametral differences in the observation group were all lower than those in the control group, with statistically significant differences (all P < 0.05). The excellent and good rate in the observation group was higher than that in the control group (P < 0.05). Conclusion The therapeutic effect of needle knife combined with manipulation and muscle strength training on chronic sacroiliac joint dysfunction is effective. It can help to correct disorders, relieve pain, improve function and maintain stability.

Clinical effect of acupotomy combined with neuromuscular electrical stimulation in the treatment of foot varus after stroke

Objective To observe the clinical effect of acupotomy combined with neuromuscular electrical stimulation in the treatment of foot varus after stroke. Methods From June 2018 to December 2019, 60 stroke patients with foot varus who were admitted to the ward of Acupuncture Rehabilitation Section One, the Third Affiliated Hospital of Anhui University of Chinses Medicine were selected. According to the random number table method, they were divided into the treatment group and the control group, with 30 patients in each group. The control group was treated with neuromuscular electrical stimulation, while the treatment group was treated with acupotomy on the basis of the control group, Both groups received one course of treatment. The simplified Fugl-Meyer assessment scale for motor function, foot varus symptom score table and maximum walking speed of 10 meters (10MWS) were used to evaluate the improvement of foot varus. Results Before treatment, there were no statistically significant differences between the two groups in the scores of simplified Fugl-Meyer assessment scale for motor function, the score of foot varus symptom and 10MWS (all P > 0.05). After treatment, the scores of simplified Fugl-Meyer assessment scale for motor function and 10MWS of the two groups were both increased compared with those before treatment, and the treatment group was higher than the control group, the differences were statistically significant (P < 0.05 or P < 0.01). After treatment, the score of foot varus symptom of two groups were both decreased compared with those before treatment, and the treatment group was lower than the control group, the differences were highly statistically significant (P < 0.01). Conclusion Acupotomy combined with neuromuscular electrical stimulation can effectively improve the symptoms of foot varus in stroke patients and speed up the rehabilitation process of patients.

Effect of small needle knife relieve sinew and regulate spur pathological jiejing points combined with sodium hyaluronate injection in the treatment of knee osteoarthritis

Objective To observe the clinical effect of small needle knife relieve sinew and regulate spur pathological jiejing points combined with sodium hyaluronate injection in the treatment of knee osteoarthritis. Methods A total of 120 patients with knee osteoarthritis admitted to Qinhuangdao Traditional Chinese Medicine Hospital, Hebei Province from January 2018 to January 2020 were selected and divided into A, B and C groups according to the random number table method, with 40 patients in each group. During the treatment, 15 patients fell off, including eight patients in group B and seven patients in group C, and 105 patients were finally treated. Group A was treated with small needle knife relieve sinew and regulate spur pathological jiejing points combined with sodium hyaluronate injection. Group B was treated with small needle knife relieve sinew and regulate spur pathological jiejing points. Group C was treated with sodium hyaluronate injection. Small needle knife treatment was performed once a week for four times in total, and intra-articular injection of sodium hyaluronate was performed once a week for five times in total. Visual analogue scale (VAS) score, WOMAC osteoarthritis index score, and clinical efficacy of the three groups were compared before and after treatment. Results WOMAC osteoarthritis index and VAS score after treatment were both lower than those before treatment, and WOMAC osteoarthritis index and VAS score in group A were lower than those in group B and C, and group B was lower than that in group C, with statistically significant differences (all P < 0.05). The total effective rate of group A was significantly higher than that of group B and group C, and the total effective rate of group B was significantly higher than that of group C, with statistically significant differences (all P < 0.05). Conclusion Small needle knife relieve sinew and regulate spur pathological jiejing points combined with sodium hyaluronate injection into articular cavity in the treatment of knee osteoarthritis is effective.

Analysis on the treatment of acute severe organophosphorus pesticide poisoning complicated with respiratory failure

Objective To analyze and discuss the treatment of acute severe organophosphorus pesticide poisoning complicated with respiratory failure. Methods A total of 16 cases of acute and severe organophosphorus pesticide poisoning patients with respiratory failure who were admitted to the Emergency Intensive Care Ward of Nanjing Integrated Traditional Chinese and Western Medicine Hospital from January 2016 to November 2019 and transferred to the emergency room. The recommendations of comprehensive treatments were provided based on use of gastric lavage, timely application of cholinoceptor blocking drugs and cholinesterase reactivators, use of tracheal intubation for patients with respiratory failure, prevention of pulmonary infections, intermediate syndrome and other complications, timely hemoperfusion therapy. Results The treatments were detailed, integrated and given to patients in accordance with their conditions. Among the total of 16 patients, 11 patients of them were cured and discharged. The duration of hospital stays ranged from 2 to 29 days with an average of 17 days. Conclusion The timely and effective removal of gastrointestinal poisons, timely performance of tracheal intubation, standardized use of ventilator to assist breathing, early, adequate and reasonable application of anticholinergics, symptomatic and comprehensive measures such as supportive treatment, are all important parts of successful treatments for reference by relevant scholars.

Application comparison of dural penetrating epidural block and direct epidural block in labor analgesia

Objective To compare the application effect of dural penetrating epidural block and direct epidural block in labor analgesia. Methods From January 2019 to January 2020, 112 parturients who need labor analgesia in Huai’an Maternal and Child Health Hospital of Jiangsu Province were selected as study objects. They were divided into group A and B according to random number table method, with 56 parturients in each group. Group A was received direct epidural block analgesia and group B was received dural penetrating epidural block analgesia. The changes of each index were monitored between two groups. Results There were no significant differences in labor stages and neonatal Apgar score between two groups (P > 0.05). There was no significant difference in visual analogue scale (VAS) score between two groups before analgesia (P > 0.05), after analgesia, VAS scores at each time point in group B were lower than those in group A, and the differences were statistically significant (P < 0.05). The levels of serum pain mediators 5-hydroxy tryptamine, substance P and neuropeptide in the first, second and third labor stages in group B were lower than those in group A, with statistically significant differences (P < 0.05). The rate of good analgesia in group B was higher than that in group A, and the difference was statistically significant (P < 0.05). There was no significant difference in the incidence of adverse events and delivery mode between two groups (P > 0.05). The labor satisfaction of group B was higher than that of group A, and the difference was statistically significant (P < 0.05). Conclusion Compared with direct epidural block, the application value of dural penetrating epidural block in labor analgesia is relatively ideal.

Influence of positive psychological suggestion and doula accompanying childbirth on psychological stress response and self-efficacy of puerpera

Objective To explore the effect of positive psychological suggestion combined doula delivery on psychological stress response and delivery efficacy of primipara. Methods From July 2018 to June 2019, 168 primiparas were admitted to the Affiliated Huai’an No.1 People’s Hospital of Nanjing Medical University. The patients were divided into the experimental group (January 2019 to June 2019) and the control group (July 2018 to December 2018), with 86 patients in the experimental group and 82 patients in the control group. The control group was given routine nursing and doula delivery companion, and the experimental group was combined with positive psychological suggestion and doula accompaniment. Psychological stress response, labor self-efficacy, labor process, postoperative bleeding 2 h, delivery mode and delivery outcome were compared between the two groups. Results After intervention, the self-rating anxiety scale (SAS) and self-rating depression scale (SDS) in the two groups were lower than those before intervention, and the scores of SAS and SDS in the experimental group were lower than those in the control group (P < 0.01). After intervention, the self-efficacy scores of the two groups were higher than those of the same group before intervention, and the results of the experimental group were higher than those of the control group in terms of efficacy, expected efficacy and total score of self-efficacy (P < 0.01). The duration of labor in experimental group 1 and 2 were shorter than that in control group, and the amount of bleeding 2 h after delivery was less than that in control group (P < 0.01); cesarean section rate was lower than that of the control group (P < 0.05), and Appar score of newborns was higher than that of the control group (P < 0.01). Conclusion Positive psychological suggestion combined with doula is helpful to relieve the psychological stress response of first-time mothers and promote the development of self-efficacy in childbirth. It has positive application value in shortening the labor process, reducing the amount of bleeding and improving the outcome of childbirth.

With the great change of modern informatization, the cultivation methods and modes of traditional Chinese medicine talents are in urgent need of keeping pace with the times. Modern teaching concepts and methods must be introduced into the classroom and teaching process to adapt to the learning concepts of students in the new era and meet the needs of social and economic development. Mind mapping as a new form of learning has been widely used, especially combined with the use of Internet technology, so that its role to a higher level. In this paper, combined with the classroom teaching practice of natural pharmaceutical chemistry, the application characteristics of mind mapping in the pharmaceutical major of natural pharmaceutical chemistry are systematically summarized, a relatively complete and mature teaching mode and method are formed, and a mind mapping knowledge system is established to improve students’ autonomous learning ability as the main line. It is hoped to improve the ability of students majoring in pharmacy to study autonomously and cultivate more high-quality pharmaceutical talents for the society to serve the health of society and human beings.
