4 Situation analysis of the national natural science foundation of China′s bone science in colleges and universities from 2010 to 2019

Objective To explore the general situation, discipline distribution characteristics, and major research directions of orthopedics in 2019 funded by the national natural science foundation of China in Chinese colleges and universities from 2010 to 2019, so as to provide reference for accelerating the further development of orthopedics research. Methods Data on the official website of national natural science foundation of China and “the annual statistics of the projects funded by national natural science foundation of China from 2010 to 2019” was screened and statistically analyzed. The situation of orthopedic funding and the distribution of research direction of orthopedics in colleges and universities in China was statistically analyzed using statistical grouping method and time series analysis method. Results According to the data, from 2010 to 2019, the funding of colleges and universities showed a curve upward trend. In 2019, national natural science foundation of China mainly focused on the research of spine, joint and bone diseases. The overall layout of the country in the field of orthopaedics showed a trend of unbalanced regional development. Conclusion Bone science is an important part of the medical field. It is suggested that the state continues to increase discipline funding, integrate resources, cultivate first-class innovative talents, improve the scientific and technological influence of orthopedics colleges and universities in China, and puts forward relevant policy suggestions for accelerating the development of orthopedics in China.

8 Effect of intraocular injection of ozonated saline on rabbit VX-2 tumor by evaluation of ultrasound elastography

Objective To evaluate the effect of different times of intraocular injection of ozonized saline on VX-2 tumor in rabbits by ultrasound elastography. Methods A total of 48 New Zealand tumor-bearing white rabbits were randomly divided into four groups by labeling. Among them, 12 rabbits were in the G0 group (injected with low-temperature normal saline, once), while 36 rabbits of the experimental group were given twice the volume of ozonized normal saline under ultrasound guidance, and according to the number of injections, they were divided into G1 (one time), G2 (two times, once a day), G3 group (three times, once a day), with 12 rabbits in each group. The long diameter (a), short diameter (b) and elasticity value (B/A) of the tumor were measured before operation, four and eight days after surgery. The tumor volume (V) was calculated by the formula V=1/2ab2 and tumor growth rate were compared. Results The tumor growth rate of the G1, G3 group four days after surgery were lower than that of the G0 group, and the differences were statistically significant (P < 0.05); the tumor growth rate of G1, G2 and G3 groups were lower than that of the G0 group eight days after surgery, and the differences were statistically significant (P < 0.05). Four days after surgery, the elasticity value of the tumor tissues in the four groups were statistically significant (P < 0.05); among them, the elasticity value of the VX-2 tumor tissues in the G1, G2 and G3 groups were lower than that in the G0 group, and the differences were statistically significant (P < 0.05). Eight days after surgery, the elasticity value of the tumor tissues in the four groups were statistically significant (P < 0.05); among them, the elasticity value of the VX-2 tumor tissues in the G1, G2 and G3 groups were lower than that in the G0 group, while the G2 and G3 groups were lower than that in the G1 group, and G3 group was lower than G2 group, and the differences were statistically significant (P < 0.05). Conclusion Two-dimensional ultrasound combined with elastography can more objectively and comprehensively evaluate the therapeutic effect of different times of intraocular injection of the same concentration of ozonated saline on rabbit VX-2 tumors, and the most obvious decrease in tumor tissue elasticity value after multiple treatments.

12 Protective effect of Shegan Mahuang Decoction on lung tissue of rats with chronic obstructive pulmonary disease and regulation of EGFR/PI3K signaling pathway

Objective To study the effect of Shegan Mahuang Decoction on endothelial growth factor receptor (EGFR)/phosphatidylinositol-3 kinase (PI3K) signaling pathway and airway smooth muscle protection in rats with chronic obstructive pulmonary disease. Methods According to the random number table method, 75 SD rats were divided into five groups, namely, the control group, the model group, the low-dose, medium-dose and high-dose Shegan Mahuang Decoction groups, 15 rats in each group. Except for the control group, all the other rats established the rat model of chronic obstructive pulmonary disease. The rats in the low-dose, medium-dose and high-dose Shegan Mahuang Decoction groups were given 10, 20, 40 g/kg Shegan Mahuang Decoction by gavage respectively, once per day. Eight weeks later, the levels of tumor necrosis factor-α (TNF-α), interleukin (IL)-6 and IL-1β in bronchoalveolar lavage fluid, lung index, the lung tissue superoxide dismutase (SOD) activity and malondialdehyde (MDA) level, histopathological changes of lung tissue and airway tissue in rats, PI3K, EGFR mRNA levels and PI3K, EGFR protein levels of lung tissue were measured. Results Compared with the model group, the levels of TNF-α, IL-6 and IL-1β in bronchoalveolar lavage fluid, lung index, the levels of MDA, PI3K, EGFR mRNA and PI3K, EGFR protein in lung tissue were significantly decreased, SOD activity in lung tissue was significantly increased (P < 0.05), and histopathology of lung tissue and airway smooth muscle was significantly improved. Conclusion Shegan Mahuang Decoction can significantly reduce the level of inflammatory factors in bronchoalveolar lavage fluid and improve the oxidative stress injury of lung tissue and histopathological changes of airway smooth muscle in rats with chronic obstructive pulmonary disease. The mechanism may be related to the regulation of EGFR/PI3K signaling pathway.

17 Effect of Zhushui Yin on rats with malignant pleural effusion and its effect on expression of TGF-β1 and AQP-1 in rat pleural tissues

Objective To investigate the therapeutic effect of Zhushui Yin on malignant pleural effusion in rats and its effect on expression of transforming growth factor-β1 (TGF-β1) and aquaporin-1 (AQP-1) in rat pleural tissues. Methods The rat model of malignant pleural effusion was established, and the successfully modeled rats were divided into five groups according to the random number table method, namely, the model group (MC), the positive control group (PC), the high-dose group (HD), the middle-dose group (MD), and the low-dose group (LD), with ten rats in each group. Another ten normal rats were recorded as normal group (NC). On the first day after modeling, PC was intraperitoneally injected with 2 mg/kg Cisplatin Injection once every two days. HD, MD and LD were respectively given 66.0, 33.0 g/kg and 16.5 g/kg of Zhushui Yin Liquid once a day. NC and MC were given the same volume of normal saline, once a day, for seven days in total. The amount of pleural effusion and the content of cancer cells in each group were compared. Pleural lesions were observed by hematoxylin-Eosin staining. The TGF-β1 and AQP-1 expression were compared in each group. Results The amount of pleural effusion and the number of cancer cells of MC, PC, HD, MD and LD on the fourth day and the eighth day were all higher than those on the first day, and the eighth day was higher than the fourth day. On the first day, the pleural effusion amount of MC rats was higher than NC, on the fourth day and the eighth day, the pleural effusion amount and cancer cell number of MC rats were higher than those of NC, while PC, HD, MD and LD were lower than MC, HD was lower than PC, LD was higher than PC, and the differences were statistically significant (all P < 0.05). The pleural structure of NC rats was normal. The pleural thickening and infiltration of cancer cells and lymphocytes were serious in MC rats. The pleural thickness in HD, MD, LD was reduced and the infiltration of cancer cells was less. Compared with NC, the expressions of TGF-β1 and AQP-1 in MC were increased (P < 0.05). Compared with MC, the expression levels of TGF-β1 and AQP-1 in PC, HD, MD and LD were all decreased (P < 0.05). Compared with PC, the expression levels of TGF-β1 and AQP-1 in HD were decreased, while the expression levels of TGF-β1 and AQP-1 in LD were increased, with statistically significant differences (all P < 0.05). Conclusion Zhuishui Yin has a good effect on malignant pleural effusion in rats, which may be related to down-regulation of TGF-β1 and AQP-1 expression.

22 Correlation between T-wave changes of avL lead with cardiovascular event endpoints in patients with acute coronary syndromes

Objective To study the correlation between T-wave changes of avL lead with the endpoints of major adverse cardiovascular events in patients with acute coronary syndrome (ACS). Methods A total of 163 cases with ACS patients who treated in Chenzhou No.1 People′s Hospital of Hu′nan Province from March 2015 to March 2018 were selected as the study objects. According to whether the endpoints of major adverse cardiovascular events occurred, they were divided into the good prognosis group (101 cases) and the poor prognosis group (62 cases). The relationship between the change of T-wave changes of avL lead and other related indexes was compared between the two groups. Results The incidence rate of T-wave changes of avL lead, ST segment elevation, ST segment depression and pathological Q wave in the poor prognosis group were higher than those in the good prognosis group (all P < 0.05). The results of predicting the endpoints of major adverse cardiovascular events by T-wave changes of avL lead were highly consistent with the follow-up results (M = 0.064, Kappa = 0.759, P < 0.05). The sensitivity and specificity were 91.93% and 86.14%, and the positive predictive value and negative predictive value were 80.28% and 94.57%, respectively. Logistic regression analysis showed that T-wave changes of avL lead, pathological Q, wave ST segment elevation and acute myocardial infarction (AMI) were independent risk predictors of the endpoints of major adverse cardiovascular events in patients. Conclusion The T-wave changes of avL lead in the body surface electrocardiogram of patients with ACS has a high predictive value for the end point of adverse cardiovascular events and is an independent risk predictor of major adverse cardiovascular events.

26 Reconstruction of the anterior talofibular ligament and strengthening of the retinaculum extensorum in the treatment of chronic lateral ankle instability

Objective To evaluate the clinical effect of small incision reconstruction of the anterior talofibular ligament combined with extensor support band enhancement in the treatment of chronic anterolateral instability of the ankle joint. Methods A total of 42 patients with chronic ankle instability from January 2016 to June 2018 were collected from the 900th Hospital of Joint Logistics Support Force. After ankle arthroscopy was conducted, reconstruction of the anterior talofibular ligament through a small incision and reinforcement of the extensor support band were performed. Preoperative and postoperative ankle function scores were compared to evaluate the therapeutic effect of this operation. Results The postoperative ankle functional scores of 42 patients was significantly higher than that before surgery, and the difference was highly statistically significant (P < 0.01). No recurrence of ankle instability was found during the follow-up period, and no patient still felt unstable after chief complaint. Conclusion Anterior talofibular ligament reconstruction and extensor band reinforcement can improve ankle instability, reduce the recurrence of ankle instability, and improve the function of ankle. It is an effective operation to treat lateral ankle instability.

30 Research progress of acupuncture brain effect based on fMRI

Acupuncture points can activate specific brain functional areas, but the mechanism is not completely clear. Objectify using functional magnetic resonance imaging (fMRI) technology, microization and other advantages, to understand the changes of the central nervous system caused by acupuncture, at the same time provides neuroimaging evidence for how acupuncture interacts in the brain. In recent years, the research on brain acupuncture effect based on fMRI technology at home and abroad was reviewed, it is found that the current research directions mainly focus on acupuncture specificity, effect mechanism, analgesia and qi extraction. This paper summarizes some of the hot topics in the current research on the combination of acupuncture and fMRI, and finds that in the brain function test, there are many interfering factors that cannot be standardized, which remains the main problem to be studied in the future. At the same time, whether the activation effect can be detected from a more microscopic and specific angle with the help of modern technology can become a direction for our joint efforts in the future.

34 Research progress of TAM receptors and its signaling pathways in immune regulation

TAM receptor is a subfamily of receptor tyrosinase, including three members Tyro3, Axl and Mertk. The common ligands of the TAM receptors are growth arrest specific gene 6 (Gas6) and S protein (ProS1). TAM receptors and ligands are widely expressed in mammalian immune, blood, reproductive, circulatory and nervous systems, and have many biological functions, such as inhibiting cell apoptosis, promoting cell proliferation, mediating cell adhesion, migration, phagocytic apoptotic cells, and participating in immune regulation. TAM receptors andits ligands play an important role in autoimmune diseases.

38 The progress in research methods in molecular biology of nosocomial infection

The epidemiological investigation of nosocomial infection pathogens is crucial to establish reasonable infection control measures. In this paper, the progress of molecular biological methods in the epidemiological investigation of nosocomial infection pathogens is described, and the application of some molecular biological methods in the epidemiological investigation of nosocomial infection outbreaks is listed. These molecular typing methods can be categorized into two main groups: DNA-based methods, such as amplified fragment length polymorphism, plasmid fingerprinting, polymerase chain reaction, pulsed field gel electrophoresis and DNA sequencing technology; protein-based methods, such as mutilocus enzyme electrophoresis and immunoblotting. Both kinds of methods have advantages and disadvantages. Therefore, which typing method is employed in practice should be selected based on the situation. It is very important for nosocomial infections to use a reasonable molecular biological typing method.

42 Research progress on pathogenesis and drug intervention of oral squamous cell carcinoma

Oral squamous cell carcinoma (OSCC) is an epithelial malignant tumor, which mainly occurs in maxillofacial skin and oral mucosa. The formation of OSCC results from multiple gene mutations, but its pathogenesis has not been fully elucidated. The mutations of p16, p53, c-erbB-2 and Bmi-1 genes are closely related to the occurrence and development of OSCC. Traditional Chinese herbal medicine extracts, such as wogonin, curcumin, licorice chalcone, and the immunosuppressant drug rapamycin have certain inhibitory effect on OSCC. This article reviews the pathogenesis of OSCC and drug intervention, which provides a theoretical basis for the research and treatment of OSCC.

46 Study on the application of oral motor intervention in the nursing of premature infants

Oral motor intervention is a method of sensory stimulation for sucking-swallowing-breathing related tissues or muscles, including oral stimulation and non-nutritional sucking. Due to the small gestational age, the physiological function of premature infants is not fully developed, and there are some deficiencies in the original reflex, it is easy to have the oropharyngeal dysfunction, which aggravates the difficulty of clinical nursing work. Therefore, appropriate interventions are urgently needed to improve the overall development of preterm infants. The application of oral motor intervention in oral feeding, growth and neurobehavioral development of premature infants is reviewed, the development prospect of preterm infant nursing was summarized in order to provide reference for further research.

50 Research progress of Gemcitabine in combined therapy of pancreatic cancer

Pancreatic cancer is one of the most aggressive tumors at present. Because of its occult incidence, most patients have been diagnosed as advanced stage and lost the opportunity of operation. Therefore, chemotherapy has become the main treatment for advanced pancreatic cancer. Gemcitabine has become the first-line chemotherapy drug for pancreatic cancer because of its unique mechanism, low toxicity, and no cross resistance when combined with other chemotherapy drugs. Due to the limited efficacy of Gemcitabine alone, and the problems of large dose use of side effects and drug resistance, domestic and foreign scholars continue to explore the combination of Gemcitabine and Gemcitabine nano carrier drug delivery technology for improving the survival period and quality of life of patients with pancreatic cancer.

54 Research progress of pyroptosis and PCSK9 in atherosclerosis

Atherosclerosis (AS) is the pathological basis of a variety of cardiovascular and cerebrovascular diseases, while seriously threatens human health and safety. The pathogenesis of AS is not yet fully understood. The latest research found that the occurrence and development of AS is closely related to the proprotein convertase subtilisin kexin type 9 (PCSK9) and pyrolysis. This article reviews PCSK9 and pyrolysis in atherosclerosis and the clinical application progress of PCSK9 inhibitors.

58 Effect of air pollutant exposure on the number of children hospitalized for common respiratory diseases

Objective To understand the impact of air pollutant exposure on the number of children hospitalized for common respiratory diseases. Methods The clinical data of pediatric hospitalized children for common respiratory diseases from January 2009 to December 2016 in Guizhou Provincial People′s Hospital and air pollutant monitoring data at various environmental monitoring points in Guiyang City including: PM2.5, PM10, nitric oxide (NO) and the concentration of sulfur dioxide (SO2) were collected. A typical correlation analysis method was used to analyze the impact of air pollutants on the number of children hospitalized for common respiratory diseases. Results A total of 10 876 hospitalized children with respiratory diseases were included in the study, and the diagnosis was mainly bronchopneumonia (76.31%), with an average hospital stay (9.56±6.05) d. The correlation coefficient between air pollutants and bronchial pneumonia was the highest (r = 0.33, P < 0.001), followed by bronchitis (r = 0.21, P < 0.001) and acute tonsillitis (r = 0.20, P < 0.001). PM10 was the main air pollutant component that affected the number of children for common respiratory diseases. Air pollution one day before hospitalization had the greatest impact on the number of children for common respiratory diseases. Conclusion Ambient air pollutants can affect the number of children hospitalizations for common respiratory diseases, of which the number of hospitalizations for bronchopneumonia is the greatest. Among them, PM10 is the most common pollutant that affects the number of children hospitalizations for common respiratory diseases, and the largest effect is one day before hospitalization.

62 Investigation of psychological resilience and analysis of related factors in patients with permanent cardiac pacemaker implantation

Objective To explore the psychological resilience and to analyze the influencing factors in patients with permanent cardiac pacemaker implantation. Methods From March to December 2019, a total of 125 patients with permanent cardiac pacemaker implantation or follow-up clinic in a top three hospital in Jiangxi Province were investigated by using the psychological resilience scale to investigate their general data and psychological resilience by convenience sampling method. Multiple linear regression analysis was used to analyze the related influencing factors of patients with permanent cardiac pacemaker implantation. Results The total scores of 125 patients with permanent cardiac pacemaker implantation were (58.93±12.96) points, and the scores of each dimension from high to low were toughness, self-reliance and optimism. Age, education level, monthly family income, children′s situation, payment methods of medical expenses, and implantation time were the influencing factors of psychological resilience in patients with permanent cardiac pacemaker implantation (P < 0.05). Conclusion Patients with permanent cardiac pacemaker implantation have a low level of psychological resilience. In the clinical work, different scientific and effective intervention programs should be targeted to improve the patient′s level of psychological resilience and promote their physical and mental recovery.

66 Construction and implementation of the health promotion activity plan of the “campus food education and practice project”

Objective To explore the “campus food education and practice project” of Liangxiang No.6 Middle School to construct and implement a health promotion activity plan for junior high school students and parents throughout the school. Methods Relying on the “campus food education and practice project”, this study was constructed feasible health promotion activities for all students and parents of Liangxiang No.6 Middle School. A total of seven activity modules including diet health literacy survey, diet survey, traditional Chinese medicine constitution determination, on-site nutrition consultation, body composition, health lectures and public account construction were implemented as planned from July 2018 to March 2019. The effectiveness of the implementation was evaluated. Results The students and parents of Liangxiang No.6 Middle School had a higher satisfaction rate in participating of health promotion activities and the event content. Among them, the “24-hour diet survey” and “computer expert system software analysis of nutrition and diet management system of traditional Chinese medicine combining with Western medicine” participated by food education association students were the most satisfied (100%). Conclusion The establishment of the “campus food education and practice project” health promotion activity program has certain feasibility, but there are still unsatisfactory aspects in the implementation, especially for the traditional Chinese medicine constitution survey and the survey data feedback need to be continuously improved.

70 Observation on the application effect of unified standardization continuity management mode for type 2 diabetes

Objective To analyze the impact of unified and standardized continuity management mode on behavior change and blood glucose control effect of patients with type 2 diabetes (T2DM). Methods From November 2017 to November 2018, 220 patients with T2DM who were treated at the National Metabolic Management Center (MMC) of the Second Affiliated Hospital of Kunming Medical University were selected. According to the random number table method, they were randomly divided into control group and experimental group, with 110 cases in each group. The control group implemented a routine management model, and the experimental group performed unified and standardized continuous management (MMC management mode). Blood glucose, self-management level and nursing satisfaction were compared before and after management. Results After intervention, the levels of glycosylated hemoglobin (HbA1c) and postprandial blood glucose (PBG) in the two groups were lower than those before intervention, fasting glucose (FPG) in the experimental group was lower than those before intervention, and FPG, PBG and HbA1c in the experimental group were lower than those in the control group, with highly statistically significant differences (all P < 0.01). Scores of rational diet, exercise, blood glucose self-test and standardized drug use in the two groups were higher than those before intervention, and scores of rational diet, exercise, blood glucose self-test and standardized drug use in the experimental group were higher than those in the control group, with statistically significant differences (all P < 0.05). The nursing satisfaction of the experimental group was higher than that of the control group, and the difference was statistically significant (P < 0.05). Conclusion The MMC management mode helps to improve the compliance rate of FPG, PBG, and HbA1c in T2DM patients, it can effectively improve the self-management behavior, increase patients′ satisfaction, and has clinical application value.

Methods From October 2017 to April 2019, the clinical data of 127 elderly patients with T2DM who failed to respond to Metformin treated in Huazhong University of Science and Technology Hospital were retrospectively analyzed. According to the treatment methods, the patients were divided into group A (40 cases treated with Insulin Ditel), group B (41 cases treated with Lialuru Peptide) and group C (46 cases treated with Insulin Ditel combined with Lialuru Peptide). The changes of body mass index (BMI), fasting plasma glucose (FPG), 2 h postprandial blood glucose (2hPBG), glycosylated hemoglobin (HbA1c), insulin resistance index (HOMA-IR) among the three groups were compared before and after treatment and the adverse reactions were compared among three groups. Results There were no significant differences in BMI, FPG, 2hPBG, HbA1c and HOMA-IR among the three groups before treatment (P > 0.05). After treatment, BMI, FPG, 2hPBG, HbA1c and HOMA-IR in the three groups were lower than those before treatment, and BMI, FPG, 2hPBG, HbA1c and HOMA-IR in group C were lower than those in group A and group B after treatment, with statistically significant differences (all P < 0.05). There was no significant difference in the incidence of adverse reactions among the three groups (P > 0.05). Conclusion In the elderly T2DM patients with Metformin-resistant, the combination of Insulin Ditel and Lialuru Peptide can significantly reduce BMI and improve the index of glucose metabolism, the effect is better than that of using alone, and it is safe.

78 Diagnostic efficacy of combined detection of serum sTNFR-1 and KIM-1 levels in type 2 diabetic nephropathy

Objective To explore the diagnostic value of serum soluble tumor necrosis factor receptor-1 (sTNFR-1) and kidney injury molecule-1 (KIM-1) levels in type 2 diabetic nephropathy (DN). Methods From January 2016 to January 2019, a total of 131 patients with type 2 diabetes mellitus in the First People′s Hospital of Kashgar ( hereinafter referred to as “our hospital”) were selected as the experimental group. According to the urinary microalbumin/creatinine ratio (ACR) level, they were divided into three groups: A1 group (n = 49), A2 group (n = 44), A3 group (n = 38). And healthy people in the same period in our hospital were selected as the control group (n = 30). The differences in general information, clinical laboratory indicators and serum STNFR-1 and KIM-1 among the groups were compared. The receiver operating characteristic (ROC) curve was used to evaluate the diagnostic value of serum sTNFR-1 and KIM-1 in type 2 diabetic nephropathy. Results There was significant difference in age among A1, A2 and A3 stages in the experimental group (P < 0.05), and the age of the A3 group was greater than that of the A2 and A1 groups, and the age of the A2 group was greater than that of the A1 group (P < 0.05). The fasting blood-glucose (FBG), ACR, sTNFR-1 and KIM-1 levels in the experimental group were significantly higher than those of the control group, and gradually increased with the increase of stages of DN, while estimation of glomerular filtration rate (eGFR) decreased gradually (all P < 0.05). sTNFR-1 and KIM-1 were positively correlated with serum creatinine (SCr), urine microalbumin (UmAlb), ACR (r = 0.475, 0.468, 0.526, 0.713, 0.693, 0.714, all P < 0.05), but negatively correlated with eGFR (r = -0.569, -0.721, all P < 0.05). The diagnostic efficacy of combination of serum sTNFR-1 and KIM-1 in the diagnosis of DN was better than that of serum sTNFR-1 and KIM-1 alone. Conclusion The serum levels of sTNFR-1 and KIM-1 in patients with DN are significantly increased and positively correlated with the severity of the disease. The detection of serum sTNFR-1 and KIM-1 in diabetic patients is helpful to the early diagnosis and early clinical intervention of DN patients and further improving the long-term prognosis of type 2 diabetic nephropathy.

82 Analysis of risk factors of postoperative infection of tibial fracture treated with locking plate

Objective To explore the related risk factors of postoperative infection of tibial fracture treated by locking plate, and to provide a theoretical basis for the prevention of postoperative infection of tibial fracture treated by locking plate. Methods A total of 416 cases of tibial fracture treated by locking plate internal fixation and followed up in the Orthopaedic Clinical Center of Gansu Hospital of Traditional Chinese Medicine from December 2017 to January 2019 were analyzed retrospectively. The incidence of infection was recorded, and the data of patients were analyzed to explore the causes and prevention methods of infection after the treatment of tibial fracture with locking plate. Results A total of 416 patients were followed up for 6 to 12 months, 45 of them were infected, 56 strains of pathogens were isolated, 34 of which were gram-positive, accounting for 60.71% of the total distribution of pathogens, mainly Staphylococcus aureus and Staphylococcus epidermidis, 22 of which were gram-negative, accounting for 39.29%, mainly Pseudomonas aeruginosa and Escherichia coli. The results of single factor analysis showed that the elderly (χ2=21.025, P < 0.001), body mass index (χ2=6.265, P = 0.012), diabetes mellitus (χ2=65.590, P < 0.001), hypertension (χ2=24.547, P < 0.001), hyperlipidemia (χ2=40.045, P < 0.001), operation time≥140 min (χ2=7.018, P = 0.008), postoperative medication time (χ2=9.502, P = 0.002), hospital stay ≥15 d (χ2=9.890, P = 0.002), intraoperative blood loss (χ2=6.460, P = 0.011), fracture types (χ2=17.104, P < 0.001), invasive operation (χ2=5.657, P = 0.017) were potential risk factors of postoperative infection. Logistic regression analysis showed that BMI≥30 kg/m2(P = 0.006), diabetes mellitus (P < 0.001), hospital stay ≥15 d (P = 0.002), open fracture (P < 0.001), invasive operation (P = 0.020) were independent risk factors for postoperative infection. Conclusion Postoperative infection is related to patients′ own condition, intraoperative condition and postoperative condition. Enough attention should be paid to it during perioperative period, and effective preventive measures should be taken actively, precise treatment plan should be made, individualized treatment should be carried out, and the incidence of postoperative infection should be reduced.

Objective To observe the efficacy of kneeling and taking Futu acupoint combined with bone setting in the treatment of the syndrome of lumbar three transverse process syndrome. Methods Seventy patients with lumbar three-transverse process syndrome who were admitted to Sanhe Hospital in Hebei Province from October 2018 to October 2019 were selected as the subjects. They were divided into the observation group and the control group according to the principle of randomized control, with 35 cases in each group. The observation group was treated by kneeling Futu acupoint combined with bone setting therapy, while the control group was treated with bone-setting technique. A total of 15 days was a course of treatment. The efficacy was evaluated and compared with visual analogue scale (VAS) score, low back pain scale (JOA) and traditional Chinese medicine efficacy criteria. Results Before the treatment, the general data of the two groups were compared, the differences were not statistically significant (P > 0.05). After treatment, VAS scores in both groups were lower than those before treatment, and the observation group was lower than that in the control group (all P < 0.05). JOA scores in both groups were higher than those before treatment, and the observation group was higher than that in the control group (all P < 0.05). The total effective rate of the observation group was higher than that of the control group, with statistically significant difference (P < 0.05). Conclusion The effect of kneeling Futu acupoint combined with bone setting is more significant in the treatment of lumbar three transverse process syndrome, which is better than the single bone setting treatment.

92 Comparison of the effect of single-segment fixation with trans-injured vertebrae and short-segment fixation with trans-injured vertebrae

Objective To compare the effect of monosegmental transvertebral fixation and short segment fixation for thoracolumbar fracture. Methods From April 2017 to April 2019, 60 consecutive patients with thoracolumbar fracture treated by operation in Beijing Luhe Hospital, Capital Medical University were selected as the research objects, and they were divided into experimental group and control group according to the random number table method, with 30 patients in each group. Experimental group accepted the injured vertebral single segmental fixation, the control group accepted across vertebral short segmental fixation, respectively in postoperative three days to begin for at least 12 months of follow-up, patients with follow-up indicators including visual analogue scale (VAS) score, Oswestry disability index scores (ODI), injury spinal vertebral compression rate, sagittal Cobb Angle, the surgery postoperative clinical curative effect and correct degree of the two operations were compared. Results At the end of follow-up, there were 28 cases in the experimental group and 25 cases in the control group. At the last follow-up, the vertebral compression rate and the sagittal Cobb angle of the two groups were significantly lower than those before operation (P < 0.05), and the experimental group was significantly lower than the control group (P < 0.05). Compared with before operation, the VAS scores and ODI scores in the last follow-up of both groups were reduced (P < 0.05), and there was no statistically significant difference between the two groups (P > 0.05). The operation time of the experimental group was significantly shorter than that of the control group (P < 0.05), and there was no statistically significant difference between the two groups in terms of intraoperative blood loss and average length of stay (P > 0.05). There were no serious complications such as wound infection, delayed nerve function damage in both groups. In the experimental group, there was no screw or titanium rod bending fracture, and screw loosening occurred in two cases. In the control group, screw fracture occurred in one case and screw loosening occurred in three cases. Conclusion Satisfactory vertebral height, sagittal Cobb angle and function are achieved by both monosegmental transvertebral and short segment fixation in treating A1 and A3.1 thoracolumbar fractures. Monosegmental transvertebral fixation is superior to short segment fixation in A1 and A3.1 thoracolumbar fractures in correction of deformity, maintenance and operating time.

96 Effect of Nickel-titanium memory alloy plate open reduction and internal fixation in the treatment of thoracic trauma with multiple rib fractures

Objective To study the effect of Nickel-titanium memory alloy plate open reduction and internal fixation in the treatment of thoracic trauma with multiple rib fractures. Methods From December 2015 to September 2019, 55 patients with chest trauma and multiple rib fractures in the Department of Thoracic Surgery of 902th Hospital of People′s Liberation Army were selected as the subjects. They were divided into study group (35 cases) and control group (20 cases). The control group was treated with traditional open reduction and internal fixation, while the observation group was treated with Nickel-titanium memory alloy plate. The effect of operation, operation related indexes and postoperative complications were compared between the two groups. Results The total effective rate of operation in the study group was significantly higher than that in the control group, the difference was statistically significant (P < 0.05). The amount of intraoperative blood loss in the study group was less than that in the control group, and the operative time, extubation time, hospital stay time and fracture healing time were shorter than those in the control group, with statistically significant differences (all P < 0.05). The incidence of postoperative complications in the study group was significantly lower than that in the control group, the difference was statistically significant (P < 0.05). Conclusion Nickel-titanium memory alloy can effectively improve the effect of treatment and reduce the incidence of surgical complications when it is used to treat patients with thoracic trauma and multiple rib fractures.

100 Correlation study on kidney size and various growth indexes in fetuses of different gestational ages

Objective To investigate the correlation on kidney size and various growth indexes in fetuses of different gestational ages. Methods A total of 127 normal single pregnancy in General Hospital of Ningxia Medical University from August 2018 to February 2019 were selected as study objects. According to gestational age, they were divided into 24-27 week group (19 cases), 28-31 weeks group (22 cases), 32-35 weeks group (44 cases), 36-40 weeks group (42 cases). Ultrasonography was used to measure fetal biparietal diameter (BPD), head circumference (HC), abdominal circumference (AC), femoral length (FL), longitudinal and transverse diameters of both kidneys. The correlation between kidney size and fetal growth indexes were analyzed. Results There was no significant difference in longitudinal and transverse diameters of both kidneys among four groups. The kidney size was positively correlated with gestational age, BPD, HC, AC and FL, and all were linearly correlated. Conclusion The measurement of fetal kidney size can be used as a reliable indicator to judge whether fetal kidney develops normally. By analyzing the relationship between fetal kidney and each growth index can understand the development of kidney laterally, understand the overall development of fetus comprehensively, and judge whether fetal development is unbalanced.

103 Meta-analysis of Magnesium Sulfate to prevent shivering during cesarean section

Objective To evaluate the effect of Magnesium Sulfate to prevent shivering during cesarean section. Methods Databases including PubMed, The Cochrane Library (Issue 8, 2019), EMbase, Wanfang Data Knowledge Service Platform (WanFang Data), China National Knowledge Infrastructure (CNKI), China Biology Medicine disc (CBM) were searched for computer to collect the randomized controlled trails (RCT) about the effect of Magnesium Sulfate on shivering for cesarean section. The retrieval time limit from repository to August 2019. The literature was tracked based on the references included in the RCT. Meta-analysis was conducted using RevMan 5.3 software, GRADE of evidence quality was used to evaluate the evidence level of meta-analysis results. Results Finally, 15 references were included, including 1043 patients and 532 patients with Magnesium Sulfate. The results of meta-analysis showed that: the incidence of shivering in experimental group was lower than that in control group (RR = 0.48, 95%CI:0.37-0.63, P < 0.000 01). The incidence of shivering in intravenous subgroup was lower than that in control group (RR = 0.46, 95%CI:0.33-0.66, P < 0.0001); the incidence of shivering in subarachnoid injection subgroup had no statistically significant difference compared with control group (P = 0.12); the incidence of shivering in epidural injection subgroup was lower than that in control group (RR = 0.38, 95%CI:0.21-0.67, P = 0.0009). The incidence of shivering in subarachnoid anesthesia subgroup was lower than that in control group (RR = 0.53, 95%CI: 0.36-0.78, P = 0.001); the incidence of shivering in combined lumbar and epidural anesthesia subgroup was lower than that in control group (RR = 0.45, 95%CI: 0.29-0.68, P = 0.0002). Meta-analysis results for different administration subgroups were all evaluated as “Moderate”. Conclusion Intravenous injection and epidural injection of Magnesium Sulfate can prevent shivering in cesarean section, but the effect of subarachnoid injection of Magnesium Sulfate to prevent the occurrence of shivering in cesarean section remains to be evaluated.

109 The value of interleukin-6 and procalcitonin to predict bacterial infection in low birth weight neonates

Objective To explore the value of interleukin-6 (IL-6) and procalcitonin (PCT) to predict bacterial infection in low birth weight neonates, and provide reference to prevent and timely treatment of neonatal infection. Methods From March 2015 to March 2017, 69 neonates with low birth weight in Department of Pediatrics, Fengxian District Central Hospital of Shanghai were selected as the low birth weight neonate group, and 30 normal neonates delivered in the delivery room during the same period were selected as the normal group. According to clinical symptoms, the low birth weight neonate group were divided into suspected bacterial infection subgroup (17 cases), sepsis subgroup (6 cases) and non-infection subgroup (46 cases). White blood cell (WBC), neutrophil (Neu), C-reactive protein (CRP), PCT and IL-6 levels were measured in the normal group and the low birth weight neonate group within 4 h after admission (after admission); WBC, Neu, CRP, PCT and IL-6 levels after admission and PCT and IL-6 levels within 4 h after treatment (after treatment) were measured between different subgroups. The predictive value of ROC curve in detecting low birth weight neonates sepsis. Results The levels of PCT and IL-6 in the low birth weight neonate group after admission were higher than those in the normal group, and the differences were statistically significant (all P < 0.05). WBC, Neu and CRP levels of the two groups were not statistically significant (P > 0.05). After admission, PCT and IL-6 levels were compared among three subgroups, and the differences were statistically significant (all P < 0.05); while there were no significant differences in WBC, Neu and CRP levels among three subgroups (P > 0.05). After treatment, there were no significant differences in PCT and IL-6 levels among three subgroups (P > 0.05). ROC curve showed that the optimal critical values of PCT and IL-6 for predicting in low birth weight neonate sepsis were 2.15 ng/L and 12.34 pg/mL, sensitivity was all 83.3%, specificity were 81.0% and 71.4% respectively, and the sensitivity and specificity of the combined test were 87.3% and 83.3% respectively. Conclusion Both PCT and IL-6 test and their combined test have certain accuracy in predicting low birth weight neonate sepsis, among which combined test has the best value.

113 Effect of exogenous growth hormone on the outcomes of IVF-ET for polycystic ovary syndrome patients

Objective To investigate the effect of exogenous growth hormone (GH) pretreatment on the outcomes of in vitro fertilization-embryo transfer (IVF-ET) in patients with polycystic ovary syndrome (PCOS). Methods From July 2016 to August 2018, 120 cases with PCOS who undergoing their first IVF-ET in the Department of Reproduction Center of Shenzhen Maternal & Child Healthcare Hospital Affiliated to Southern Medical University were selected as objects. The patients were divided into treatment group (68 patients) and control group (52 patients) according to whether exogenous GH was used during ovulation induction. The laboratory indicators and clinical outcomes between two groups were compared. Results The total dosage of gonadotropins and the days of gonadotropins stimulation in treatment group were lower than those in control group (all P < 0.05), while endometrial thickness on the day of ovum pick-up, the normal fertilization rate, high-quality embryo formation rate, clinical pregnancy rate and live birth rate in treatment group were higher than those in control group (all P < 0.05). There were no statistically significant differences between the two groups in the incidence of moderate to severe ovarian hyperstimulation syndrome, the number of retrieved oocytes, the cleavage rate, embryo transfer day intrauterine membrane thickness and abortion rate (P > 0.05). Conclusion The pretreatment with exogenous GH helps to improve the outcome of IVF-ET in PCOS patients.

117 Clinical study of Xiaoer Jiebiao Oral Liquid combined with Xiaoer Paracetamol Huangnamin Granules in the treatment of exogenous fever in children

Objective To investigate the clinical efficacy and safety of Xiaoer Jiebiao Oral Liquid combined with Xiaoer Paracetamol Huangnamin Granules in the treatment of exogenous fever in children. Methods From July 2018 to April 2019, 230 children with exogenous fever who admitted to Binzhou People′s Hospital of Shandong Province were selected as objects. According to the random number table method, they were divided into control group (115 cases) and observation group (115 cases). Control group was treated with Xiaoer Paracetamol Huangnamin Granules, while observation group was treated with Xiaoer Jiebiao Oral Liquid combined with Xiaoer Paracetamol Huangnamin Granules. Both groups were given continuous treatment for three days. The clinical efficacy was evaluated after treatment. The antipyretic onset time, antipyretic time and complete antipyretic time of two groups were compared. The TCM syndrome scores and serum tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β), γ interferon (IFN-γ) and C-reactive protein (CRP) levels were compared between two groups before and after treatment. The incidence of adverse reactions during the treatment was recorded and compared between two groups. Results The total clinical effective rate of observation group was higher than that of control group (P < 0.01). The antipyretic onset time, antipyretic time and complete antipyretic time of observation group were shorter than those of control group (all P < 0.01). There was no statistically significant difference in TCM syndrome scores between two groups before treatment (P > 0.05); TCM syndrome scores in two groups after treatment were lower than those before treatment, and observation group was lower than control group (all P < 0.01). Before treatment, there was no significant difference in serum TNF-α, IL-1β, IFN-γ and CRP levels between two groups (P > 0.05); after treatment, the levels of serum TNF-α, IL-1β, IFN-γ and CRP in two groups were lower than those before treatment, and observation group was lower than control group (all P < 0.05). During the treatment period, no serious adverse reactions occurred in both groups. Conclusion Xiaoer Jiebiao Oral Liquid combined with Xiaoer Paracetamol Huangnamin Granules has definite clinical efficacy in treating exogenous fever in children. It can shorten the antipyretic time, improve clinical symptoms and reduce the level of serum inflammatory factors, and has good safety.

121 Analysis of postpartum venous thrombosis in Yangzhou Maternal and Child Gare Service Centre from 2012 to 2018

Objective To investigate the incidence, risk factors, occurrence time, clinical manifestations and diagnosis of postpartum venous thrombosis, and to avoid overtreatment. Methods The clinical data of five cases of postpartum venous thrombosis in 47 948 pregnant women admitted to Yangzhou Maternal and Child Care Service Centre (hereinafter referred to as “our hospital”) from January 2012 to December 2018 were retrospectively analyzed. The score according to the criteria set out in the April 2015 Royal College of Obstetricians and Gynaecologists guidelines on reducing the risk of venous thromboembolism during pregnancy and puerperium, the differences in clotting function before delivery and at the time of thrombus discovery were compared. Meanwhile, the coagulation function was compared with that of 6525 normal pregnant women without complications and complications in the third trimester pregnancy in our hospital in 2018. Results All five cases were confirmed by color doppler ultrasonography or CT pulmonary angiography in general hospitals and our hospital, including three cases of deep vein thrombosis, two cases of superficial vein thrombosis, and one case of deep vein thrombosis. In recent six years, the incidence was (1.04-1.07)/10 000. All of the five cases occurred within one week after delivery. Among the five patients, one case had a risk factor score of four, one case had a risk factor score of three, and three cases had a risk factor score of one. Four cases had lower extremity pain symptoms. Comparison of coagulation function between the five patients before delivery and at the time of thrombosis discovery showed no statistically significant difference (P > 0.05). However, compared with the coagulation function before delivery of 6525 normal pregnant women, the prothrombin time (PT) and thrombin time (TT) of five cases before delivery were shorter than that of normal pregnant women (P < 0.05). There were no significant differences in activated partial prothrombin time, international standardized ratio, fibrinogen and D-dimer (P > 0.05). After anticoagulant therapy, all the five cases recovered. Conclusion The incidence of postpartum venous thrombosis is low, PT and TT before delivery are shortened significantly and lower limb pain symptoms appear after delivery.

Objective To investigate the clinical application effect of oral appliance in patients with obstructive sleep apnea-hypopnea syndrom (OSAHS) combined with laryngopharyngeal reflux (LPR). Methods From August 2016 to January 2018, 46 patients diagnosed as OSAHS in the Department of Otolaryngology Head and Neck Surgery, the First Affiliated Hospital of Guangzhou Medical University were selected to receive six-month oral appliance for intervention. Polysomnography monitor and Dx-pH monitoring were performed before and after six months of the intervention respectively, and reflux symptom index (RSI) and reflux fineling score (RFS) were completed. Sleep snoring, sleep apnea hypopnea index (AHI), 24 h pH monitoring, symptoms related to LPR, RSI and RFS were compared before and after intervention. Results Before intervention, there were seventeen patients with RSI more than thirteen points and/or RFS more than seven points, while only three cases after six months of intervention. Before intervention, there were eleven patients with positive Ryan index in orthostatic and/or decubitus positions, while only two cases after six months of intervention. The AHI, snoring index, RSI, Ryan index in orthostatic position and Ryan index in decubitus position of patients were significantly lower than those before intervention, and mean oxygen saturation was significantly increased, the differences were highly statistically significant (P < 0.01). Scores of multiple RSI items (persistent throat clearing, excessive sputum or nasal reflux, dyspnea or recurrent asphyxia, throat foreign body sensation) and RFS items (laryngeal mucus adhesion, diffuse laryngeal edema) were significantly lower than those before intervention, and the differences were highly statistically significant (P < 0.01). Conclusion Oral appliance is helpful for the treatment of OSAHS patients combined with LPR and can significantly relieve the symptoms of patients with LPR.

129 Effect of pterygium excision autologous limbal stem cell transplantation combined with mitomycin C in the treatment of pterygium

Objective To investigate the effect of pterygium excision combined with autologous limbal stem cell transplantation and mitomycin C in the treatment of patients with pterygium, and to provide a reference for clinical treatment selection. Methods A total of 104 pterygium patients (135 eyes) admitted to the Department of Ophthalmology, Traditional Chinese Medicine Hospital of Lu′an from January 2017 to February 2019 were selected as study subjects. According to the simple number random grouping method, they were divided into the observation group with 52 cases (66 eyes) and the control group with 52 cases (69 eyes). The control group was treated with pterygium excision autologous limbal stem cell transplantation, while the observation group was treated with pterygium excision autologous limbal stem cell transplantation combined with mitomycin C, follow-up for 12 months. Clinical efficacy, rehabilitation, corneal astigmatism, complications and recurrence were compared between the two groups. Results The total effective rate of the observation group was significantly higher than that of the control group, with statistically significant difference (P < 0.05). There was no significant difference in the repair time of corneal epithelium, the disappearance time of ocular irritation and the healing time of grafts between the two groups after surgery(P > 0.05). Three months after surgery, the corneal astigmatism of both groups was significantly decreased compared with that before surgery, and the observation group was significantly lower than the control group, with statistically significant differences (all P < 0.05). There was no significant difference between the two groups in the complication rate within 3 months after surgery (P > 0.05). The total recurrence rate in the observation group was significantly lower than that in the control group, with statistically significant difference (P < 0.05). Conclusion The curative rate of pterygium excision combined with autologous limbal stem cell transplantation and mitomycin C in patients with pterygium is high, the improvement of corneal astigmatism is good, the postoperative recurrence rate is low, and the application value is high.

Objective To establish a quality control method for Ma′nao Gao. Methods Thin layer chromatography was applied to the qualitative identification of Faeces Togopteri, Corydalis Rhizoma, Rhei Radix et Rhizoma, Psoraleae Fructus, Notoginseng Radix et Rhizoma. High performance liquid chromatography was used for the content determination of tetrahydropalmatine, acetonitrile-0.1% triethylamine phosphate system was used as the mobile phase of gradient elute. The column temperature was 30°C and the detection wavelength was set at 280 nm. Results Thin layer chromatography spots was clear, which had good resolution without interference. Tetrahydropalmatine had good linear relationship with the range of 2.022-20.22 μg/mL (r = 0.9999) respectively, the average recovery was 100.27% (RSD = 2.39%). Conclusion The method is reliable with good stability and good repeatability, and can be used for quality control of Ma′nao Gao.

137 Clinical observation of Lipoic Acid combined with Pancreatic Kininogenase Enteric-Coated Tablets in the treatment of type 2 diabetic peripheral neuropathy

Objective To compare the therapeutic effect of Lipoic Acid and Lipoic Acid combined with Pancreatic Kininogenase Enteric-Coated Tablets (Yikai) on type 2 diabetic peripheral neuropathy. Methods From January 2018 to January 2019, 60 patients with diabetic peripheral neuropathy were hospitalized in the Department of Endocrinology, the Affiliated Huai′an No.1 People′s Hospital of Nanjing Medical University were enrolled. The patients were divided into the observation group and the control group by random number table method, with 30 patients in each group. The control group was given intravenous infusion of Lipoic Acid, and the observation group was given intravenous infusion of Lipoic Acid combined with Yikai oral administration for two weeks. The clinical efficacy was evaluated, electromyography, oxidative stress and inflammation indexes of the two groups before and after treatment were evaluated respectively. Results After treatment, the total effective rate of the observation group was higher than that of the control group (P < 0.05). Sensory conduction velocity (SNCV) and motor conduction velocity (MNCV) of the common peroneal and median nerves in the two groups were not significantly different from those before treatment (P > 0.05). After treatment, SNCV and MNCV of the common peroneal nerve and median nerve in both groups were higher than those before treatment, and SNCV and MNCV of the observation group were higher than those of the control group, with statistically significant differences (all P < 0.05). Malondialdehyde (MDA), reactive oxygen species (ROS), advanced oxidizing protein products (AOPPs) and superoxide dismutase (SOD) in the two groups were not significantly different before treatment (P > 0.05). After treatment, MDA, ROS and AOPPs in both groups were lower than those before treatment, SOD was higher than that before treatment, SOD in the observation group was higher than that in the control group, while MDA, ROS and AOPPs were lower than those in the control group, with statistically significant differences (all P < 0.05). Comparison of hypersensitive C-reactive protein (hs-CPR), interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) before treatment between the two groups showed no significant difference (P > 0.05). After treatment, hs-CPR, IL-6 and TNF-α in the two groups were all lower than those before treatment, and hs-CPR, IL-6 and TNF-α in the observation group were lower than those in the control group, with statistically significant differences (all P < 0.05). Conclusion Intravenous infusion of Lipoic Acid combined with Yikai oral administration can effectively improve the clinical symptoms of patients with type 2 diabetes peripheral neuropathy, which may be related to reducing oxidative stress and improving the state of micro-inflammation. It plays a good synergistic effect and is worthy of clinical promotion.

Although rectovaginal fistula and flatus vaginalis are two distinct concepts of Western medicine and traditional Chinese medicine, they have similar symptoms. The reason is due to the adjacent anatomical relationship. Flatus vaginalis is conscious or there is indeed vaginal gas excretion, and the main manifestation of rectovaginal fistula is vaginal exhaust excretion. There′re similarities and differences among them, so this paper will introduce the rectovaginal fistula and flatus vaginalis separately, and finally take summary analysis of the relationship between them. From this conclusion, rectovaginal fistula can be manifested as flatus vaginalis when the fistula is small, which may be formed by many factors, rectovaginal fistula is only one of them. For flatus vaginalis caused by rectovaginal fistula, both doctors and patients should jointly decide whether to choose conservative treatment or elective surgery.

Ménière disease belongs to the category of “otogenic vertigo” in traditional Chinese medicine. It is a common type of peripheral vertigo in clinical practice. Professor He Weiping combined with clinical experience to discuss from the perspective of the spleen and stomach. All diseases are caused by the weakness of spleen and stomach. Dysfunction of spleen and stomach, lost source of blood and qi, and dereliction of rise and fall duty are the main causes and pathogenesis of Ménière disease. It is suggested that Ménière disease should be treated by conditioning the spleen and stomach. In the acute phase, the principle of treatment is to strengthen the spleen and stomach, unblock stuffy orifice and to relieve dazzle, while in the intermittent phase, the method is to take care of the spleen, stomach and restore the balance of viscera, yin and yang. Sijunzi Decoction, Fuzi Lizhong Decoction, Buzhong Yiqi Decoction, Banxia Baizhu Tianma Decoction, Linggui Zhugan Decontion are selected according to syndrome types such as weakness of the spleen and stomach, deficient cold of spleen and stomach, lack of qingyang, wind-phlegm disturbing upward, and deficiency of zhongyang. In order to regulate the spleen and stomach, it will also take into account the integrity of the internal and external environment of the body, and focus on guiding patients to adjust their eating habits, regulate their emotions and moderate work and rest, etc. The clinical effect is remarkable and with one case as an example. It hopes to provide some experience and reference to for the treatment of Ménière disease.

153 Clinical effect of Gualou Xialing Decoction in the treatment of stable angina pectoris with coronary artery heart disease with syndrome of phlegm blocking heart vessel

Objective To observe clinical efficacy of Gualou Xialing Decoction in the treatment of stable angina pectoris of coronary artery heart disease with syndrome of phlegm blocking heart vessel. Methods From January 2018 to June 2019, 110 patients with stable angina pectoris of coronary artery heart disease with phlegm blocking heart vessel syndrome treated in Ji′nan Municipal Hospital of Traditional Chinese Medicine were selected and randomly divided into control group (50 cases) and treatment group (60 cases) by using complete randomization method. Control group was given conventional Western medicine treatment, and treatment group was given Gualou Xialing Decoction on the basis of control group. After four weeks of treatment, comprehensive curative effect, total curative effect of traditional Chinese medicine syndrome, the rate of nitroglycerin stopping and decreasing, blood homocysteine (Hcy) level, hypersensitive C-reactive protein (hs-CRP) level, fasting blood glucose, fasting insulin and insulin resistance index (HOMA2-IR) of two groups were observed. Results Total effective rate between the two groups had statistically significant difference(P > 0.05). Total effective rate of traditional Chinese medicine syndrome in treatment group was higher than that in control group (P < 0.01). The stopping and decreasing rate of nitroglycerin in treatment group was significantly higher than that in control group (P < 0.05). After treatment, hs-CRP and Hcy in two groups were lower than those before treatment (P < 0.05), and treatment group was lower than that in control group (P < 0.05). After treatment, fasting insulin and HOMA2-IR in treatment group were lower than those before treatment and those in control group (P < 0.05). Conclusion Gualou Xialing Decoction combined with Western medicine has a significant effect on stable angina of coronary artery heart disease. It can significantly improve the clinical symptoms, reduce the use of nitroglycerin, reduce the level of Hcy and hs-CRP in the blood of patients, and improve the insulin resistance of patients.

157 Clinical research of Xueshuan Tongmai Decoction combined with acupuncture in the treatment of cerebral apoplexy

Objective To explore clinical efficacy of Xueshuan Tongmai Decoction combined with acupuncture in the treatment of cerebral apoplexy. Methods One hundred and twenty patients with cerebral apoplexy admitted to Affiliated Hospital of Shandong Academy of Medical Sciences from January 2017 to January 2019 were selected. According to the random number table method, the patients were divided into routine group, acupuncture group and combined group, with 40 cases in each group. Routine group was treated with routine Western medicine. Acupuncture group was treated with acupuncture on the basis of routine group. Combined group was treated with Xueshuan Tongmai Decoction on the basis of acupuncture group. All three groups were treated for 3 months. Clinical efficacy was evaluated after treatment. The score of Chinese medicine symptoms, National Institute of Health stroke scale (NIHSS), Glasgow coma scale (GCS), serum nitric oxide (NO) and endothelin (ET-1) levels were compared before and after treatment. The incidence of adverse reactions during the treatment was recorded and compared among three groups. Results Total clinical effective rate of combined group was higher than that of routine group and acupuncture group (P < 0.05). After treatment, the scores of Chinese medicine symptoms (numbness of limb, spontaneous sweating, panting and weakness, dark tongue quality) in three groups were lower than those before treatment (P < 0.05); after treatment, the scores of Chinese medicine symptoms in acupuncture group and combination group were lower than those in routine group (P < 0.05); after treatment, the scores of Chinese medicine symptoms in combination group were lower than those in acupuncture group (P < 0.05). NIHSS scores of three groups after treatment were lower than those before treatment, GCS scores of three groups after treatment were higher than those before treatment (P < 0.05); NIHSS scores of acupuncture group and combination group after treatment were lower than those of routine group, GCS scores of acupuncture group and combination group after treatment were higher than those of routine group (P < 0.05); NIHSS score of combined group after treatment was lower than that of acupuncture group, GCS score of combined group after treatment was higher than that of acupuncture group (P < 0.05). After treatment, serum NO levels in three groups were higher than those before treatment, and serum ET-1 levels in three groups were lower than those before treatment (P < 0.05); serum NO levels in acupuncture group and combination group after treatment were higher than those in routine group, and serum ET-1 levels in acupuncture group and combination group after treatment were lower than those in routine group (P < 0.05); after treatment, serum NO level in combined group was higher than that in acupuncture group, and serum ET-1 level in combined group was lower than that in acupuncture group (P < 0.05). There were no serious adverse reactions during the treatment of three groups. Conclusion Xueshuan Tongmai Decoction combined with acupuncture has a significant clinical effect in treating cerebral apoplexy, which can significantly improve neurological function and vascular endothelial function damage of patients, and it has good safety.

Objective To observe clinical efficacy of Jiawei Zhenwu Decoction combined with Levothyroxine Sodium Tablets in the treatment of hypothyroid heart disease with syndrome of yang deficiency of heart and kidney. Methods Fifty patients with hypothyroid heart disease with syndrome of yang deficiency of heart and kidney from October 2016 to October 2019 in Department of Cardiology, Affiliated Hospital of Shaanxi University of Chinese Medicine were selected and divided into control group and treatment group by random number table method, each with 25 cases. Control group was given conventional Levothyroxine Sodium Tablets, while treatment group was given Jiawei Zhenwu Decoction on the basis of control group. After 12 weeks of treatment, traditional Chinese medicine syndrome efficacy, thyroid function, cardiac function and safety of two groups were compared before and after treatment. Results Total effective rate of traditional Chinese medicine syndrome in treatment group was higher than that in control group (P < 0.05). After treatment, the scores of traditional Chinese medicine symptoms in two groups were lower than those before treatment, and treatment group was lower than that in control group (P < 0.05). After treatment, the levels of free thyroxine, free triiodothyronine, thyroxine and triiodothyronine in two groups were higher than those before treatment, and the level of thyroid stimulating hormone was lower than that before treatment, and the improvement of various indexes in treatment group was more obvious than that in control group (P < 0.05). After treatment, the left ventricular ejection fraction of two groups were higher than that before treatment, the cardiac output and stroke volume were both higher than those before treatment, the heart rate was faster than that before treatment, and the improvement degree of each index in treatment group was more obvious than that in control group (P < 0.05). There was no significant difference in the incidence of adverse reactions between two groups (P > 0.05). Conclusion The effect of Jiawei Zhenwu Decoction combined with conventional Western medicine Levothyroxine Sodium Tablets in the treatment of hypothyroid heart disease is remarkable. The combination of traditional Chinese and Western medicine can significantly improve heart function, thyroid function and reduce clinical symptoms.

165 Clinical study of traditional Chinese medicine massage combined with Huatan Tongluo Jiejing Decoction in the treatment of spastic paralysis after stroke

Objective To explore clinical effect of traditional Chinese medicine massage combined with Huatan Tongluo Jiejing Decoction in the treatment of spastic paralysis after stroke. Methods One hundred and six patients with spastic paralysis after stroke treated in People′s Hospital of Jianyang City from September 2018 to June 2019 were divided into control group (n = 53) and observation group (n = 53) by random number table method. Two groups were given routine treatment. Control group was treated with rehabilitation and massage therapy of traditional Chinese medicine. Observation group was treated with rehabilitation and massage therapy of traditional Chinese medicine and Huatan Tongluo Jiejing Decoction. Clinical efficacy, serum glutamate (Glu), gamma aminobutyric acid (GABA), modified Ashworth scale (MAS), fugl meyer motor function scale (FMA), Barthel index (BI) and patient satisfaction were compared between two groups. Results Total effective rate in observation group was higher than that in control group, the difference was statistically significant (P < 0.05). After treatment, Glu level in two groups was lower than that before treatment, and GABA level in two groups was higher than that before treatment, Glu level in observation group was lower than that in control group, and GABA level in observation group was higher than that in control group (P < 0.05). After treatment, the scores of MAS, FMA and BI in two groups were higher than those before treatment, and the scores in observation group were higher than those in control group (P < 0.05). Total satisfaction of observation group was higher than that of control group (P < 0.05). No adverse reactions occurred in two groups during the treatment. Conclusion Traditional Chinese medicine massage combined with Huatan Tongluo Jiejing Decoction has a significant effect in the treatment of spastic paralysis after stroke, which can effectively improve the level of serum amino acid neurotransmitters.

The treatment emphasizes distinguishing false and true, treating both symptoms and root causes, treating by stages. During the activity period, the main treatment is to dispel pathogenic qi, the key is to clear away heat, remove dampness and toxin, and the treatment will be based on this theory after toxin; during the stable period, the main treatment is to strengthen the thealthy qi, and the key is to harmonize the liver, spleen and kidney so as to restore the patient′s healthy qi and improve the human body′s immunity. It is based on the empirical Modified Huangjing Decoction. At the same time, we should pay attention to the combination of nourishing, the treatment of heart and liver, the coordination of body and spirit, the overall treatment, and the clinical effect is more prominent. A classic case is attached to prove its effectiveness.

173 Treatment of SAPHO syndrome with combination of Chinese and Western medicine: a case report

Synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome is a type of rare disease involving skin, bone and joint lesions. Its skin lesions are characteristic, including palmoplantar pustular disease, pustular psoriasis, fulminant acne, acne conglobata and hidradenitis suppurativa, but skin manifestations are not necessary. Osteoarticular changes in the middle sternal rib clavicle area of bone fat thickness and bone sclerosis, that is, “tau sign” has a high degree of characteristics. Due to the characteristics of SAPHO syndrome, such as high misdiagnosis rate, long diagnosis time, and no recognized diagnosis and treatment guidelines, SAPHO syndrome has caused huge burden and trouble to patients with this disease. The diagnosis and treatment based on TCM syndrome differentiation can give patients with SAPHO syndrome a significant effect in controlling disease activity and improving quality of life. This paper reports a case of SAPHO syndrome treated with traditional Chinese and Western medicine and reviews related literature, so as to increase the understanding of SAPHO syndrome, at the same time to explore the advantages of TCM in treating this disease.

Objective To observe the impact of standardized grading transfer program on nosocomial transfer of critically ill patients in department of emergency obstetrics and gynecology, and to provide a reference for the smooth implementation of standardized grading transfer program. Methods A cluster sampling method was used to retrospectively analyze the clinical data of 200 patients with critical gynecology and obstetrics in Beijing Obstetrics and Gynecology Hospital, Capital Medical University (hereinafter referred to as “our hospital”) from January to December 2017, as the control group. Retrospectively analyze the clinical data of 220 gynecological and critically ill patients after the standardized grading transfer program were carried out in our hospital from January to December 2018, as the observation group. The incidence of adverse events during the transfer, the time of completion of the transfer, and nursing satisfaction were compared between the two groups. Results The incidence of falling bed, vaginal bleeding, worsening of the condition and abnormal fetal movement during the transportation of the observation group were significantly lower than those of the control group, and the differences were statistically significant (P < 0.05). The various time for completing the transfer in the observation group were significantly shorter than those in the control group, and the differences were statistically significant (P < 0.05). The nursing satisfaction of the observation group was significantly higher than that of the control group, and the difference was statistically significant (P < 0.05). Conclusion The development and implementation of the standardized grading transfer program can reduce the incidence of adverse events in nosocomial transport of critically ill patients in the department of emergency, significantly shorten the time of transport, and improve satisfaction. It is recommended to carry out and implement nosocomial transport of critically ill patients in our hospital to improve transport safety.

It is becoming more and more serious for the aging of the population in China and is urgent to explore and develop elderly care services. Relying on the blue Ocean strategy of traditional Chinese medicine, developing the model of traditional Chinese medicine health care service for the elderly can not only promote the development of traditional Chinese medicine, but also actively deal with the problem of rapid aging in China. This article adopts the method of case analysis, to select the typical cases under different health care model of traditional Chinese medicine, analysis the current status of development of the health care senice of fusion of traditional Chinese medicine and traditional Chinese medicine health of the main problems in the elderly care service model, and in accordance with this, the reasonable suggestions on the development of traditional Chinese medicine health care are put forward.

193 Research on the existing problems and discussion on countermeasures of poverty alleviation by health

Poverty and return to poverty due to illness ranks first among all causes of poverty, which has become the difficulty of the current work of poverty alleviation. This article takes the survey results of two counties in Xinyang City, He′nan Province as an example, to introduce the main measures to implement local poverty alleviation. At the same time, it compares the gaps between the health protection services of the general population and the poor, the compensation for disease expenditure, while dissects the problems caused by the measures of compensation after event in the existing health poverty alleviation work, and analyzes the causes of the poor people′s insufficient self-development ability and the impacts the effects on poverty alleviation. In addition, puts forward a few suggestions on the sustainable development of poverty alleviation: improve health-related poverty alleviation policies and measures and establish an early warning mechanism; establish a multi-funded critical illness insurance system to ease the pressure of local finance and medical insurance funds; clarify the responsibilities and obligations of the poor in health poverty alleviation; establish poverty relief groups, strengthen community guidance and organization stand by.
