GE Ling1 CHEN Hejian2 XU Hangdi3
Objective To explore the role of miR-501 in non-small cell lung cancer (NSCLC) with gefitinib resistance and its underlying mechanism. Methods The expression of miR-501 in NSCLC with gefitinib resistance serum and PC9/GR were evaluated by quantitative real-time polymerase chain reaction (qRT-PCR). The effect of miR-501 on the malignant phenotype of PC9/GR was detected by CCK-8, scratch test and Transwell test. The online database was used to predict the potential target of miR-501 and to verify its specific role in PC9/GR. Results Compared with the healthy control people and the normal lung epithelial cell lines, the expression level of miR-501 in gefitini-resistant NSCLC patients serum and PC9/GR were increased, and the differences were highly statistically significant (P < 0.01). Compared with the mimics NC, the proliferation, migration and invasion capacity of PC9/GR in the miR-501 mimics were increased, and the differences were highly statistically significant (P < 0.01). The luciferase reporter assay confirmed that BLID was a direct target for miR-501. Compared with the miR-501 mimics+pcDNA3.1, the proliferation, migration and invasion of PC9/GR in the miR-501 mimics+ pcdna3.1-blid were decreased, and the differences were highly statistically significant (P < 0.01). Conclusion The upregulation of miR-501 promoted the proliferation, migration, and invasion of geffitini-resistant NSCLC by inhibiting BLID. Therefore, miR-501 is a potential target for gefitinib-resistant NSCLC.

LI Jianwei1 ZHANG Li′na2 ZHANG Yingmin1 KANG Shaoying1 WEI Meng1
Objective To study the correlation between abnormal methylation of miR-203 gene promoter region in synovial tissues and the level of joint fluid proinflammatory factors in obese knee osteoarthritis (KOA). Methods Synovial tissue and joint fluid of KOA patients were taken from 36 patients who underwent knee arthroplasty in Handan Central Hospital in Hebei Province from January 2018 to January 2019. Patients with body mass index (BMI)≥28.0 kg/m2 were included in the obesity group (23 cases), and patients with 18.0 kg/m2≤BMI≤23.9 kg/m2 were included in the normal control group (13 cases). Methylation specific PCR (MSP) was used to detect the methylation status of the gene promoter region of miR-203 in synovial tissues. The expression level of miR-203 in synovial tissues was determined by quantitative polymerase chain reaction (qPCR). qPCR was used to detect the expression levels of miR-203 in the obese group and the normal control group. Pearson correlation analysis was used to investigate the correlation between miR-203 and BMI, tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β) and interleukin-6 (IL-6) in joint fluid. Results In the synovial tissues of the obese group, the promoter region of miR-203 gene was abnormally hypomorphic, while in the normal control group, it was partially hypomorphic. Compared with the normal control group, the expression level of miR-203 in synovial tissues of the obesity group was increased, and the difference was statistically significant (P < 0.05). The relative expressions of IL-1β, IL-6 and TNF-α in the joint fluid of the obese group were higher than those of the normal control group, with statistically significant differences (all P < 0.05). The methylation level in the promoter region of miR-203 gene was negatively correlated with population BMI and the IL-1β, IL-6 and TNF-α in the joint fluid (all P < 0.05). The expression level of miR-203 was positively correlated with population BMI and the IL-1β, IL-6 and TNF-α in the joint fluid (all P < 0.05). Conclusion Hypophalation of miR-203 gene promoter in synovial tissues of KOA patients is closely related to BMI and levels of joint fluid proinflammatory factors in obese KOA patients, and miR-203 can be used as an important regulatory factor for inflammation of obese KOA and a potential target for treatment of KOA.

Treatment progress on traditional Chinese and Western medicine of postherpetic neuralgia
HUANGFU Jiaxin SHEN Dexin
Postherpetic neuralgia (PHN) is the most common complication of acute herpes zoster. The persistence of pain has a substantial effect on the patients′ quality of life. At present, a variety of methods are used to treat PHN. Anticonvulsants, antidepressants, opioids and topical therapies including Lidocaine and Capsaicin are the most widely used therapies for the treatment of PHN, due to adverse effects, they should be used carefully in the elderly patients or with significant comorbidities. Nerve block, neuromodulation and other measures also play an important role in the treatment of PHN. However, the efficacy, tolerability and safety of these invasive methods should be closely monitored. Chinese medicine therapy have little side effects and significant effect for the treatment of PHN, we should be carried out individualized differentiation of symptoms and signs on the basis of types. For some PHN patients who have intractable pain and poor treatment effect, we can consider integrated traditional Chinese and Western medicine to give full play to their advantages. This paper reviews the traditional Chinese and Western medicine treatment of PHN, so as to provide reference for clinical treatment.

LI Dengjun1 LI Hailiang2 LIU Chenggong2 LIU Guangquan1
Objective To compare the clinical effects of suprapatellar approach and transpatellar ligament approach with intramedullary nail in the treatment of middle and lower tibial fractures. Methods A total of 100 cases of middle and lower tibial fractures admitted to Dongying People′s Hospital in Shandong Province from March 2015 to March 2018 were selected as the research object. According to the different treatment plans, the patients were divided into group A and group B, with 50 cases in each group. The group A was fixed with suprapatellar approach with intramedullary nail fixation, and the group B was fixed with transpatellar ligament approach with intramedullary nail fixation. The postoperative clinical effect, clinical related indicators, knee joint function and complications were compared between the two groups. Results The excellent and good rate of the group A at 12 months after operation was higher than that of the group B, and the difference was statistically significant (P < 0.05). The times of fluoroscopy in the group A was less than that in the group B, the time of operation and fracture healing after operation was shorter than that in the group B, and the ratio of cross-section diameter of before and after operation was lower than that in the group B, with statistically significant differences (all P < 0.05). However, there was no statistically significant difference in intraoperative blood loss between the two groups (P > 0.05). There were statistically significant differences in the comparison between groups, in the comparison at time points and in the interaction between the two groups (all P < 0.05). Compared with two weeks after operation, HSS scores of patients in the two groups and 6 and 12 months after operation were increased, and the special surgical hospital scoring system in the United States (HSS) scores of patients in the two groups 12 months after operation were higher than 6 months after operation, with statistically significant differences (all P < 0.05). Two weeks after the operation, there was no statistical difference in HSS score between the two groups (P > 0.05). HSS scores in the group A at 6 and 12 months after the operation were higher than those in the group B, with statistically significant differences (all P < 0.05). The incidence of anterior knee pain in the group A was lower than that in the group B (P < 0.05). Conclusion Suprapatellar approach is better than transpatellar ligament approach with intramedullary nail in the treatment of middle and lower tibial fracture. It has the advantages of shorter operation time and shorter healing time after operation. It is conducive to the recovery of knee joint function, and it has lower incidence of anterior knee pain after operation, so it is worthy of clinical application.

ZHANG Haisen1 LIU Chang1 LIANG Dongqi2 WANG Huailiang3 PEI Baojing3 LIU Hui4 LIU Ying5
Objective To investigate clinical effect of external fixator pinning in anterior process of calcaneus combined with mini-open reduction and internal fixation in treatment of distal tibia comminuted fracture. Methods The clinical data of 19 patients with closed distal tibial comminuted fracture who received external fixator pinning in anterior process of calcaneus combined with mini-open reduction and internal fixation in the Derpatment of Orthopedics, Cangzhou Central Hospital in Hebei Province from January 2014 to January 2016 were retrospectively analyzed. During the operation, external fixation screws of the distal end of the fractures were placed in medial posterior side of the calcaneus and about 1 cm of sustentaculum of talus of calcaneus. A limited incision was made in the anterior medial side of the ankle, and screw fixation was adopted after the anatomical reduction of the distal articular surface fracture of the tibia. External fixator was relaxed for 1 time per week after postoperative 4 weeks in order to exercise motion of ankle. The perioperative data and follow-up data were analyzed to evaluate the postoperative clinical effect. Results The mean operation time was (58.6±15.4) min, the average intraoperative blood loss was (201.5±78.6) mL, and the average hospitalization time was (14.2±2.8) d. Two cases (10.5%) had postoperative superficial inflammatory reaction around fibular incision, and one case (5.3%) had postoperative infection of external fixation screw track, which healed well after treatment. No neurological or vascular injury complications occurred in all patients. The incidence of perioperative complications was 15.8%. The patients were followed up for 13 to 15 months. All fractures were healed without loss of reduction. There was no statistically significant difference between the Lowa ankle score and the visual analogue scale pain score at each postoperative follow-up time point (P > 0.05). According to the Maryland score, the good rate of the curative effect was 84.2%. Conclusion External fixator pinning in anterior process of calcaneus combined with mini-open reduction and internal fixation in treatment of distal tibia comminuted fracture has good clinical effect, reliable fixation and low complication rate, which is an alternative surgical treatment method.

ZHU Kang WANG Xiaotong MA Hujing DU Haishan
Objective To explore the effect of Huoxue Xiuduan Decoction in the treatment of closed fractures, analyze the factors influencing the curative effect and the intervention strategies. Methods A total of 83 patients with closed fractures who were treated in Beijing Changping Hospital of Traditional Chinese Medicine from May 2016 to January 2018 were selected for observation and divided into the control group (41 cases) and the experimental group (42 cases) according to the random number table method. The control group was treated with conventional therapies such as closed reduction combined with splint external fixation, conventional antibiotics and bone protecting capsules. On the basis of the control group, the experimental group was treated with self-designed Huoxue Xiuduan Decoction. The clinical efficacy, inflammatory factors, pain and swelling degree of the two groups were observed. Logistic regression was used to test the influencing factors of the treatment effect of the experimental group. Results The total effective rate of the experimental group was higher than that of the control group, and the difference was statistically significant (P < 0.05). After treatment, the levels of interleukin-6 and interleukin-8 in the experimental group were significantly lower than those in the control group, with statistically significant differences (all P < 0.05). After 1 day of treatment, there was no statistically significant difference between the two groups in pain score and swelling degree score (P > 0.05). After 7 days of treatment, the pain and swelling degree scores of the two groups were lower than those after 1 day of treatment, and the experimental group was significantly lower than the control group, with statistically significant differences (all P < 0.05). The reasons for the ineffective treatment in the experimental group were not correlated with gender, age, and cause of injury (P > 0.05), and were correlated with the patients′ sleep time and whether they ate raw, cold, sour and spicy food (P < 0.05). Logistic regression analysis showed that sleep time < 8 h and frequent consumption of cold, sour and spicy food were risk factors for treatment failure (P < 0.05). Conclusion The effect of self-designed Huoxue Xiuduan Decoction in the treatment of closed fracture is good, and it is worth popularizing in clinic. When using this treatment, the patient should follow the doctor's advice, follow a strict diet, and get enough sleep

Objective To investigate the metabolism factors related to gestational diabetes mellitus (GDM) and/or non-alcoholic fatty liver diease (NAFLD) and diagnostic value of Triglyceride-glucose (TyG) index in pregnancy. Methods From June 2018 to September 2019, Pregnant women who labor examination and delivery in the Second Affiliated Hospital of Zhengzhou University (“our hospital” for short) were selected. Pregnant women with GDM were divided into GDM group (137 cases), NAFLD group of pregnant women with NAFLD (70 cases), pregnant women with GDM combined with NAFLD were in complex group (73 cases), the healthy pregnant women who came to our hospital for labor examination at the same time were selected as normal group (73 cases). Fasting blood glucose (FBG), fasting insulin (FINS), glycosylated hemoglobin (HbA1c), total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), aspartate aminotransferase (AST) and alanine aminotransferase (ALT), serum uric acid (SUA) in 24-28 weeks pregnancy were compared, the homeostasis model of insulin resistance index (HOMA-IR) and TyG index were calculated. Results FBG, HbA1c, HOMA-IR and TyG index in GDM group were higher than those in normal group. ATL, SUA, TG, HOMA-IR and TyG index of NAFLD group were higher than those of normal group. ATL, SUA, TG, FBG, HbA1c, HOMA-IR and TyG index in complex group were higher than those in normal group and GDM group and FBG, HbA1c, HOMA-IR and TyG index were higher than those in NAFLD group, with statistically significant differences (all P < 0.05). FINS and the incidence of macrosomia in GDM group, NAFLD group and complex group were higher than those in normal group, and the differences were statistically significant (P < 0.05). Pearson correlation analysis showed that TyG index was positively correlated with HOMA-IR and HbA1c (r = 0.473, 0.472, P < 0.001). Logistic regression analysis showed that FBG (OR = 1.707, 95%CI:1.202-2.424, P = 0.003) and TG (OR = 1.386, 95%CI:1.031-1.862, P = 0.031) were risk factors for NAFLD in pregnant women with GDM. ROC curve showed that HOMA-IR and TyG index had diagnostic value for the occurrence of GDM and/or NAFLD. Conclusion Pregnant women with GDM combined with NAFLD have severe IR and metabolic disorders, FBG and TG are involved in occurrence of NAFLD in pregnant women with GDM. Therefore, effective measures should be taken to control clinically. TyG index can initially identify IR and assess blood glucose level.

WANG Hanzhou1 ZHU Guangzhao2 YANG Yuzheng3 GAO Yang1 HAN Xiaochen4 SHI Bai5 HOU Jialun6 WANG Hailong1
Objective To evaluate the effect of Tripterygium Glycoside Tablets on the expression of pro-inflammatory cytokines and anti-inflammatory cytokines related to interleukin (IL)-23/IL-17 inflammatory axis in rheumatoid arthritis (RA) treatment. Methods Search through Chinese and English databases such as CNKI, VIP, WanFang, SinoMed, EMbase, PubMed, Clinical Trials, Web of Science, Cochrane Library, etc. With a period of time from the establishment of the databases to December 22, 2019. Randomized controlled trials (RCT) of Tripterygium Glycoside Tablets for RA were included, from which the data was extracted. Methodological quality evaluation was performed using the “bias risk” assessment tool recommended by Cochrane Collaboration, and RevMan 5.3 software was used for meta-analysis. Results Compared with using Methotrexate alone, Tripterygium Glycoside Tablets combined with Methotrexate had an advantage in reducing the levels of IL-1, IL-6 and tumor necrosis factor-α, the difference were statistically significant (P < 0.05). While in comparison with using Methotrexate or Leflunomide alone, Tripterygium Glycoside Tablets combined with Methotrexate or Leflunomide had shown no statistically marked difference regarding the effect on IL-10(P > 0.05). Conclusion Tripterygium Glycoside Tablets combined with Methotrexate or Leflunomide can significantly reduce the expression of IL-23/IL-17 axis related to pro-inflammatory cytokines. However, there are rare RCTs that have reported the effect of Tripterygium Glycoside Tablets on anti-inflammatory cytokines, and the existing evidence can not reach the conclusion that Tripterygium Glycoside Tablets can improve the level of anti-inflammatory cytokines.

Application of fuyang theory in external treatment of Chinese medicine
YE Bo1 WANG Hongxia2 ZHAO Zhihong3 FANG Jin4 LI Haixia5
The external treatment of traditional Chinese medicine is to use various external methods to apply drugs or energy to the skin, orifices, shu points and other parts, so as to play its role in clearing channels and collaterals, harmonizing qi and blood, detoxifying and removing blood stasis, strengthening the healthy and removing pathogenic factors, readjusting and improving the imbalance of yin and yang in the viscera, so as to promote the recovery of body functions and achieve the purpose of treatment. The external treatment of traditional Chinese medicine has a long history. The emergence of Li Yue Pianwen by Wu Shangxian in the Qing Dynasty marks the formation of a complete theoretical system, in which, the concept of supporting yang is embodied. The application of fuyang theory in the external treatment of traditional Chinese medicine can be divided into two categories to guide the clinical practice: first, it is mainly used for warming and removing blood stasis, including acupuncture, scraping, massage, walking on stilts, chiropractic and qigong; second, it is mainly used for warming and invigorating yang, including moxibustion, medicine bath, Chinese medicine sticking, paraffin therapy, etc.

A case of Israeli actinomycete infection and literature review with pneumonia as the main manifestation
FENG Yi1 LI Xinyang2 WANG Kaijin3▲
Israeli actinomycetes is a kind of actinomycetes, which can cause chronic suppurative granulomatous disease of the lung, forming single or multiple pulmonary nodules. It is relatively rare to cause bronchial pneumonia. In addition, the widespread application of penicillin reduces the reported cases of pulmonary actinomycosis, but it still exists in remote areas. If early diagnosis and early treatment are not possible, it may lead to to poor prognosis, lung function decline, loss of labor force, and even endanger the lives of patients, so it is crucial to raise awareness of the disease. However, the infection caused by this pathogen is mostly chronic suppurative granulomatous infection, which is mostly manifested as nodules or cavitary lesions with air floating signs, and the imaging has no specificity. Therefore, to do early diagnosis, early treatment, only to improve the awareness of the disease. The imaging manifestation of this patient is pneumonia, which is more rare. In this paper, the basic knowledge of actinomycetes, the basic information of patient, the diagnosis and treatment process, treatment prognosis, literature review and other aspects of the disease are elaborated in depth, so as to increase clinicians′ understanding of the disease.
