7 Effect of PI3K and p-Smad7 expression on proliferation of osteosarcoma cells

Objective To study the effect of PI3K and p-Smad7 expression on the proliferation of osteosarcoma cells. Methods From January 2017 to October 2018, 60 osteosarcoma patients (osteosarcoma tissues) and 60 chondroid tissues (normal tissues) from Department of Orthopedics Tangdu Hospital, Air Force Military Medical University of PLA were selected as subjects for study and immunohistochemical staining. The changes of two kinds of bone tissues and the expression of PI3K and p-Smad7 in different tissues were observed. Osteosarcoma MG-63 cells were treated with PI3K inhibitor CAL-101 (2.5 nm) as experimental group and untreated osteosarcoma MG-63 cells as control group. The proliferation capacity (MTT proliferation experiment), cloning ability (clonogenic experiment) and expression of PI3K and p-Smad7 (detection of qRT-PCR and Western blot) of MG-63 cells in two groups were observed. Results The expressions of PI3K and p-Smad7 in osteosarcoma tissues were significantly higher than those in normal tissues (P < 0.01). After administration of 48 and 72 hours, proliferation capacity of MG-63 cells in experimental group was significantly lower than that in control group (P < 0.05). After administration of 48 hours and 15 days of continuous culture, cloning ability of MG-63 cells in experimental group was significantly lower than that in control group (P < 0.05). After administration of 36 hours, expression of PI3K and p-Smad7 in MG-63 cells in experimental group was significantly lower than that in control group (P < 0.05). Conclusion Abnormal expression of PI3K and p-Smad7 interaction can promote proliferation of osteosarcoma cells.

11 Effects of curcumin on autophagy and degeneration of human intervertebral disc nucleus pulposus

Objective To investigate the effect of Curcumin on autophagy and degeneration of human intervertebral disc nucleus pulposus. Methods The human disc nucleus pulposus tissues were obtained from 7 patients with disc herniation admitted to the Department of Orthopedics, Suining Central Hospital of Sichuan Province from March to November 2018. Primary culture of nucleus pulposus cells were performed in vitro and divided into three groups, control group, interleukin-1β (IL-1β) group and interleukin-1β + Curcumin group. The toxicity of Curcumin to human nucleus pulposus cells was detected by tetramethylazoles salt colorimetry (MTT) assay; RT-PCR and Western blot were used respectively to detect each cell matrix metalloproteinases-3 (MMP-3), MMP-13, gathered proteoglycans (aggrecan) and collagen (Collagen) Ⅱ expression level; meanwhile, protein expressions of autophagy protein microtubule-related protein 3 (LC3) and Beclin 1 in each group were detected. The autophagy level in each group was observed by transfection with GFP-LC3 adenovirus. Results MTT results showed no cytotoxicity to nucleus pulposus cells when curcumin concentration was less than 80 μmol/L. Considering that it has no toxic effect on nucleus pulposus cells and can exert maximum drug effect on nucleus pulposus cells. Curcumin concentration of 80 μmol/L was selected for subsequent experiments. Curcumin can significantly increase the number of autophagy particles in nucleus pulposus cells, promote the increase of autophagy protein LC3 level and decrease of P62 protein level (P < 0.05). Curcumin significantly improve aggrecan nucleus pulposus cells and Collagen Ⅱ level, reduce the MMP-3 and the expression of MMP-13 levels (P < 0.05). Conclusion Curcumin can slow disc degeneration, possibly by promoting autophagy in cells.

16 The role of apoptosis-related proteins in pathological changes of Alzhei-mer′s disease

Objective To study the role of apoptosis-related proteins in pathological changes of Alzheimer′s disease. Methods Six senescence accelerated mouse P8 (SAMP8) aged 3 months, 12 SAMP8 aged 6 months and 6 senescence accelerated mouse/resistance 1 (SAMR1) aged 6 months were selected and divided into four groups, group A had 6 SAMR1 mice of 6-month-old, group B had 6 SAMP8 mice of 3-month-old, both groups were selected as control. Group C had 6 SAMP8 mice (sham treatment group), Group O had 6 SAMP8 mice (drug treatment group) aged 6 months. Group O was intraperitoneally injected with Obatoclax 0.2 mL, while group C was injected with the same amount of 0.9% physiological saline for 14 consecutive days. The cognitive function was tested by Morris water maze and the levels of Bcl-2, p-tau and caspase-3 were detected by immunohistochemistry. Results Compared with group A, the incubation periods of Morris water maze experiment in group B, C and O were significantly prolonged, and the times of platform crossings were significantly reduced (P < 0.05). Compared with the first experiment, the incubation period of the second Morris water maze experiment in group O was highly prolonged, and the times of platform crossings was statistically decreased (P < 0.05). Compared with group C, the number of positive cells protein of Bcl-2 in group O increased significantly (P < 0.05), but there was no significant difference in the mean optical density (IOD) and number of positive cells of caspase-3 (P > 0.05), there also was no significant difference in the IOD and number of positive cells of p-tau protein (P > 0.05). Conclusion The SAMP8 mice show a tendency of memory decline with prolonged survival time. Bcl-2 is a key component in cognitive change and cognitive change may not through pathways of the caspase-3 or p-tau.

20 Study on the analgesic effect of extracts from Weigela florida by water

Objective To explore the analgesic effect of the extracts of Weigela florida by water in mice. Methods Water extracts of Weigela florida were prepared by soak, extraction, and filtration. Animal models of pain caused by hot plate method and writhing method. A total of 150 clean-grade Kunming mice were selected, and used in both male and female, and hot plate method was used in 50 mice, they were randomly divided into small, medium and large dose groups of Weigela florida water extracts (equivalent to crude drug 0.05, 0.1, 0.2 g/kg), Tramadol Hydrochloride group (0.04 g/kg) and control group (equal volume saline), 10 mice in each group, and administered by intragastric or intraperitoneal injection. The pain thresholds of each groups at 15, 30, 45, 60, 75 min and 90 min after administration were measured. The other 100 animal models with wriggle-induced pain were treated by intragastric or intraperitoneal injection (block method was the same as before), the times of writhing response and inhibition rate of wriggle-induced pain were measured at 11-20 min (T1), >20-30 min (T2), >30-40 min (T3), and >40-50 min (T4) after administration. Results Compared with the saline group, the pain thresholds in the medium dose group at 60 and 75 min, and the large dose group at 15, 45 and 60 min increased significantly, the differences were statistically significant (P < 0.05). Compared with Tramadol Hydrochloride group, there were no statistically significant differences in the medium dose group at 45, 75 and 90 min, the large dose group at 45 and 90 min (P > 0.05), while the differences at other time points were statistically significant (P < 0.05 or P < 0.01). Compared with the saline group, the times of writhing response after the intragastric administration in the small and the large dose group at T1-T4 periods, and the medium dose group at T3 and T4 periods; the times of writhing response after the intraperitoneal injection in the small and the medium dose group at T3 and T4 periods, and the large dose group at T2, T3 and T4 periods decreased significantly, the differences were statistically significant (P < 0.05 or P < 0.01). Compared with Tramadol Hydrochloride group, the times of writhing response after the intragastric administration in the small dose group at T1 and T2 periods, the medium dose group at T1, T2 and T3 periods, the large dose group at T2 period; the times of writhing response after the intraperitoneal injection in the small dose group at T1 and T3 periods, the medium dose group at T1, T2 and T3 periods, the large dose group at T1 period were markedly increased, the differences were statistically significant (P < 0.05 or P < 0.01). Compared with the saline group, the pain threshold of the tramadol hydrochloride group was significantly increased after intragastric or intraperitoneal injection (P < 0.05 or P < 0.01), and the times of writhing response was significantly decreased (P < 0.01). Conclusion Weigela florida water extracts have certain analgesic effects on mice, and the effect is more obvious in the large-dose group. The analgesic effect of tramadol hydrochloride is clear and significant.

24 Effect of water extract of Acorus tatarinowii on the expression of nitric oxide in human neurogliocyte cell line

Objective To investigate the effect of water extract from Acorus gramineus on the expression of nitric oxide (NO) in human brain glial cells. Methods LPS was used to stimulate human brain glial cells (HEB) to produce a large amount of NO and induce pathological model. They were divided into control group (phosphate buffer), water extract of Acorus gramineus tatarinus (37.5 μg/mL group), water extract of Acorus gramineus tatarinus (75 μg/mL group), water extract of Acorus tatarinus tatarinus (150 μg/mL group), water extract of Acorus tatarinus tatarinus (300 μg/mL group), water extract of Acorus tatarinus tatarinus (600 μg/mL group), water extract of Acorus tatarinus tatarinus (1000 μg/mL group). The cell activity was detected by methyl thiazolyl tetrazolium and NO was detected by Griess. Results Compared with the control group, the water extract of acorus gramineus tatarinowii had a significant inhibitory effect on HEB cell activity, which was concentration-dependent and time-dependent. The water extract of Acorus gramineus had NO significant effect on the expression of NO in normal HEB cells, but could significantly reduce the lps-induced NO (P < 0.01). Conclusion Acorus gramineus can significantly inhibit the concentration of NO in human brain glial cells induced by LPS, thus protecting brain cells.

27 Simultaneous determination of 5 kinds of antioxidants in 3 kinds of medicinal plastic bottles by HPLC

Objective To establish a method for the determination of antioxidant BHT, 1010, 330, 1076 and 168 in medicinal plastic bottles made from polypropylene, polyethylene and polyester by high performance liquid chromatography. Methods Agilent Eclipse XDB-C18 (250 mm×4.6 mm, 5 μm) column was used as the column, methanol-water was used as the mobile phase of the gradient elute, with the detection wavelength of 278 nm and the flow rate of 1.0 mL/min. The colomn temperature maintained at 30℃. Results The antioxidant BHT, 1010, 330, 1076 and 168 in the range of 4.969-49.69 mg/L, 5.080-50.08 mg/L, 4.994-49.94 mg/L, 5.014-50.14 mg/L and 4.994-49.94 mg/L showed good linear relationship with the peak area (r > 0.999). The average contents of antioxidant 1010, 330, 1076 and 168 were respectively 31.58-203.82 μg/g, 97.26 μg/g, 56.15-214.45 μg/g and 44.68-265.09 μg/g in the plastic bottles for medicinal use produced by different manufacturers. Conclusion The method is simple and accurate, and it can be used to simultaneously determine the content of 5 kinds of antioxidants in 3 kinds of medicinal plastic bottles.

32 Research progress of stem cells in treatment of closed incomplete tendon injury

With the development of sports and national fitness, the incidence of closed incomplete tendon injuries has increased every year. At present, clinically commonly used treatment methods such as non-steroidal anti-inflammatory drugs, acupuncture physiotherapy and acoustic and optical stimulation have certain limitations. In recent years, genetic engineering and tissue engineering have been continuously developed, laying a good foundation for stem cell treatment of closed incomplete tendon injury. However, through the literatures at home and abroad, there are few special studies on the application of stem cell therapy for athletes with closed incomplete tendon injury. This paper intends to summarize the related research and provide reference for follow-up research and clinical treatment.

37 Research progress of integrated traditional Chinese and Western medicine in the treatment of hepatic fibrosis induced by chronic hepatitis B

Hepatic fibrosis (HF) is the healing reaction to liver injury and the process of overdeposition of hepatic extracellular matrix, which is a reversible pathological process from chronic liver disease to liver cirrhosis, liver biopsy is the best standard for its diagnosis. Chronic hepatitis B (CHB) is the main cause of liver fibrosis in China, the central point of liver fibrosis is the activation of hepatic stellate cells, traditional Chinese medicine believes that the interaction of "dampness, heat, toxin, blood stasis and deficiency" is the pathogenesis. The integrated therapy with traditional Chinese medicine and antiviral drugs not only embodies the characteristics of etiological treatment, but also exerts the advantages of multiple targets, multiple parts and less side effects of traditional Chinese medicine and has become a hot topic in the study of liver fibrosis. Based on the pathogenesis and diagnostic criteria of HF, the writer elaborates the clinical application of integrated therapy with traditional Chinese and Western meidicine on HF induced by CHB, aiming to provide a good method for clinical treatment of HF.

41 Research progress on gastric mucosal protection mechanism of calcitonin gene-related peptide

Calcitonin gene-related peptide (CGRP) is a predominant neurotransmitter derived from capsaicin-sensitive sensory nerves, which are widely distributed in the gastrointestinal system. These sensory nerves protect gastric mucosa against damage by sensing various stimuli, and CGRP plays an important role in this process. CGRP protects the gastric mucosa by inhibiting gastric acid secretion, increasing gastric mucosal blood flow, relieving gastrointestinal motility, anti-inflammatory and anti-apoptotic mechanisms. Increasing the release of CGRP is also considered as a potential treatment for gastric diseases. This article focuses on the biological properties of CGRP and the important repairing effect of CGRP on gastric mucosal injury, which is intended to provide new enlightenment for the treatment of gastric ulcer.

45 Advanced in research of histone lysine methyltransferase and diabetes mellitus

Diabetes mellitus is a group of chronic hyperglycemia characterized by chronic metabolic diseases caused by multiple causes. Long-term elevated blood glucose results in damage to large blood vessels and microvessels and endangers the heart, brain, kidneys, peripheral nerves, eyes, feet, etc. Many histone modifying enzymes in epigenetics play important roles in diabetes mellitus. More and more studies have confirmed the relationship between histone lysine methyltransferase and diabetes mellitus and its complications. Histone lysine methyltransferase not only affects the expression of insulin gene through the interaction with some transcription factors, but also affects the occurrence and development of diabetes by promoting the transformation of functional beta cells. Histone lysine methyltransferase is also involved in diabetic complications by affecting inflammatory cytokines, reactive oxygen species, etc. This article is a brief review of the research progress of histone lysine methyltransferase in diabetes mellitus.

With the increasing incidence of the breast cancer, it brings women not only physical injury, but also psychological damage. As a research direction in positive psychology, resilience has attracted more and more attention. Based on the introduction of concept and theoretical frame in resilience, this paper reviews questionnaires, influence factors, longitudinal research status and intervention methods in breast cancer patients. It aims to provide a reference for the resilience of breast cancer patients in the future.

53 Research progress on the regulatory mechanism of pro-mitochondrial fusion mitotic proteins in mitochondrial fusion, division and apoptosis

Mitochondria is widely found in higher animals and plants due to their specific structure and function. Mitochondria contains a large number of enzyme systems, which can be coupled to oxidative phosphorylation via an electron transport chain to form adenosine triphosphate, which is the main production structure. The realization of mitochondrial function is inseparable from the changes in its structure. Mitochondrial fusion and division are the basis for achieving function. Mitochondrial fusion can promote the connection between adjacent mitochondria, and mitochondrial division can promote the normal distribution of mitochondria in cells, which is of great significance for maintaining normal physiological activities of the body. So far, mitochondrial fusion proteins that have been extensively studied in mammals include mitofusin 1 and  mitochondrial fusion protein 2. Mitochondrial proteins include dynamin-related protein 1. The mitochondrial network structure is destroyed when the mitochondrial fusion and division movements are unbalanced, the pro-apoptotic factors such as cytochrome C are released, which leads to apoptosis. In this paper, a brief review of the regulation of mitochondrial fusion, mitochondrial fission and apoptosis was made in combination with the research results.

61 Application of flipped classroom teaching model based on WeChat and micro-class in nursing undergraduates of our school

Objective To explore the application of flipped classroom teaching model based on WeChat and micro-class in nursing undergraduates of Heilongjiang University of Chinese Medicine ("our school" for short) by taking Basic Nursing as an example. Methods The subjects of this study were nursing undergraduates of grade 2015 in our school from October 2017 to October 2018. Classes 1 and 2 were experimental group (96 students), while classes 3 and 4 were control group (95 students). Flipped classroom teaching model based on WeChat and micro-class was applied in experimental group (the design of micro-class--building of WeChat public platform--students′ autonomous learning before class--the development of classroom teaching activities--evaluation and feedback, summary and reflection), while traditional teaching mode of teachers and students was used in control group. The basic theory and skills test scores, autonomous learning ability and critical thinking ability of two groups were compared. Results Test scores of basic theory and basic skills in experimental group were significantly higher than those in control group, the differences were statistically significant (P < 0.05). The scores of truth-seeking, open thinking, analytical ability, systematization ability, self-confidence of critical thinking, curiosity and cognitive maturity in experimental group were significantly higher than those in control group, the differences were statistically significant (P < 0.05). The scores of learning motivation, planning and implementation, self-management, interpersonal communication and total scores in experimental group were significantly higher than those in control group, the differences were statistically significant (P < 0.05). Conclusion The flipped classroom teaching mode of WeChat and micro-class can effectively improve the learning effect of nursing undergraduates in our school, and help to improve students′ critical thinking ability.

65 Discussion on the teaching of molecular pathology as an elective course for medical undergraduates

Molecular pathology is a new frontier interdisciplinary subject in modern medicine. It has wide clinical practicability and is often used in clinical staging of cancer and diagnosis of infectious diseases. Developing this course in medical colleges and universities is not only to meet the needs of teachers and students, but also to conform to the development and progress of this subject. The purpose of this study is to explore the significance and teaching methods of molecular pathology elective course for medical undergraduates. The study is carried out from three aspects: the urgency of the course, the implementation at home and abroad, and the problems and solutions in the teaching process. The study is based on training guidelines formulated by the National Certification Organization for Clinical Laboratory Sciences of the United States and the cancer diagnosis and treatment standards formulated by the Chinese Society of Clinical Oncology in conjuction with teaching practice.

69 Teaching effects evaluation on a mini-course of English on issues and solutions in clinical medicine and its inspirations for ESP teachers

Objective To investigate the teaching effects of a mini-course of English on issues and solutions in clinical medicine and its inspirations for English for specific purposes (ESP) teachers. Methods Taking the mini-course of English on issues and solutions in clinical medicine as an example, a questionnaires were distributed among 110 clinical students of Xi′an Medical University in July 2018. And the questionnaire was used to investigate the students′ satisfaction with ESP and English for general purpose (EGP). Results One hundred and ten questionnaires were distributed and collected, the effective rate was 100%. Results showed more desirable effects of ESP than EGP in teaching methods, class participation and post-class self-study ability, the differences were statistically significant (P < 0.05). Conclusion The textbook of ESP fits with students′ learning needs, has with positive effects and with favorable evaluation from the students.

73 Fusion of "innovation within the framework" and education of innovation- entrepreneurship among college students

This paper combined the characteristics of innovative education in colleges and brought in the theory of "innovation within the framework" to construct guidelines of college students′ creative activities. A series of strategies such as orchestration of tasks, attributes dependency, subtraction, division and multiplication are applied within the framework. Through describing fusion and application of "innovation within the framework" and education of innovation- entrepreneurship among college students, this paper provides data support for innovation education in colleges.

79 Effect of conventional treatment assisted by Danshen Injection in the treatment of oral submucous fibrosis and its influence on SFRP1 level

Objective To explore the therapeutic effect of conventional treatment assisted by Danshen Injection in the treatment of oral submucous fibrosis (OSF) and the influence on secreted frizzled related protein 1 (SFRP1). Methods A total of 68 patients with OSF admitted to the First Hospital of Hu'nan University of Chinese Medicine from April 2016 to March 2018 were selected and divided into single drug group (34 cases) and combined group (34 cases) by using random number table method. The single drug group was treated with Triamcinolone Acetonide and the combined group was given Danshen Injection combined with Triamcinolone Acetonide. The mouth opening, oral mucosal impairment area, visual anologue scale (VAS), levels of SFRP1 in gingival crevular fluid and saliva, serum interleukin-6(IL-6), transforming growth factor-β1 (TGF-β1) and clinical effect were compared before and after treatment. Results Before treatment, there were no significant differences in oral mucosal impairment area, VAS score, levels of SFRP1, serum IL-6, TGF-β1 in gingival crevular fluid and saliva between the two groups (P > 0.05); after treatment, the oral mucosal impairment area and VAS score of the combined group were significantly lower than those of the single drug group and mouth opening was higher than that of the single drug group (P < 0.05); levels of SFRP1 in gingival crevicular fluid and saliva in the combined group was higher than that in the single drug group (P < 0.05); serum IL-6 and TGF-β1 in the combined group were lower than those in the single drug group (P < 0.05); the therapeutic effect of the combined group was significantly better than that of the single drug group (P < 0.05). Conclusion The effect of Danshen Injection combined with Triamcinolone Acetonide in the treatment of OSF is better than Triamcinolone Acetonide, which can effectively increase the levels of SFRP1 in gingival crevicular fluid and saliva, and slow down the development of oral submucous fibrosis.

83 The value of bedside ultrasound lung comets in the diagnosis of acute heart failure

Objective To explore the application value of bedside ultrasound lung comets (ULCs) in the patients with acute heart failure (AHF). Methods Forty cases of patients with AHF admitted to the Department of Emergency of Chongqing General Hospital (the Third Branch) from May 2016 to May 2017 were collected. Within 24 hours of the diagnosis, all patients were examined by the experienced examiners respectively by bedside lung ultrasound and chest CT. The correlation between pulmonary ultrasonography scores and chest CT scoresis was determined, as well as the correlation between the number of ULCs and echocardiographic parameters [left atrial diameter (LA), left ventricular end-diastolic diameter (LVEDD), ejection fraction (EF)], and the specificity and sensitivity of patients with AHF were identified by ULCs. Results Pulmonary ultrasonic scores for patients were (16.43±4.31) scores, and the chest CT scores were (12.11±5.32) scores, there was a significant linear correlation between the two scores (r = 0.561, P < 0.01). The number of ULCs was positively correlated with LA and LVEDD (r = 0.331, P < 0.01; r = 0.325, P < 0.01), and negatively correlated with EF (r = -0.378, P < 0.01). The area under the ROC curve for ULCs was 0.933 (95%CI: 0.895, 0.980). When the number of ULCs was more than 5, the sensitivity to diagnose AHF was 90.2% (37/41) and the specificity was 66.7% (6/9); when the number of ULCs was more than 8, the sensitivity to diagnose AHF was reduced to 68.3% (28/41) and the specificity was 88.9% (8/9); when the number of ULCs was greater than 20, the sensitivity of diagnosis of AHF was reduced to 51.2% (21/41) and specificity reached 100.0% (9/9). Conclusion Ultrasound lung detection of ULCs is a good method for evaluating and determining AHF. It has high sensitivity and specificity, and has broad application prospect in AHF assessment.

87 Effect of intermuscular fat content in soleus on mortality rate of COPD patients

Objective To investigate the effect of intermuscular fat content in soleus on the mortality rate of patients with chronic obstructive pulmonary disease (COPD). Methods One hundred and twelve non-hospitalized COPD patients diagnosed in the First Affiliated Hospital of Hebei North University from January 2014 to March 2015 were collected. Ntermuscular fat content, body mass index (BMI) and other relevant clinical data were collected and followed up by telephone for 3 years. Cox proportional hazard regression model was used to evaluate the relationship between all-cause mortality and BMI, intermuscular fat content of soleus muscle and other factors. Results A total of 21 patients died during the follow-up period, with older age and longer course of disease, COPD global initiative interpretation(GOLD) grade and intermuscular fat content in soleus muscle, and lower BMI and forced expiratory volume at 1 second (FEV1), and more patients with comorbidities, less patients receiving long-term home oxygen therapy. BMI < 21 kg/m2 was a risk factor for high mortality among patients with COPD in risk model Ⅰ(HR = 2.97, P = 0.02); the soleus muscle fat content > 12% was a risk factor for high mortality among patients with COPD in risk model Ⅱ(HR = 2.17, P = 0.0014). - 2 times of the logarithmic likelihood function value of risk model Ⅱ was lower than the risk modelⅠ. Conclusion BMI and intermuscular fat content of soleus have important reference value in determining the prognosis of patients with COPD at stable stage, and the latter may be superior to the former.

91 Clinical study of transesophageal echocardiography in evaluating of recurrence patients with atrial fibrillation after circumferential pulmonary vein isolation

To evaluate the changes of pulmonary vein and left atrial pulmonary vein crest width in patients with atrial fibrillation after loop pulmonary vein electrical isolation by transesophageal echocardiography. Methods Forty-eight cases of postoperative recurrence patients in the Department of Cardiology, Sanya People′s Hospital, Hainan Province were selected from October 2014 to August 2016. Changes in the diameter of the left superior, left inferior, right superior, right inferior pulmonary vein opening and left atrial pulmonary vein crest width were measured by esophageal ultrasound within 1 week before ring pulmonary vein isolation, 48 h after surgery and 5 months after surgery. Results Five months after the operation, the diameter of the left superior, the left inferior, the right superior and the right inferior pulmonary vein opening were compared with that of the 1 week before the operation and 48 h after the operation, and the difference was statistically significant (P < 0.05); 48 h after surgery, the diameter of left upper, left lower, right upper, right lower pulmonary vein opening, and the maximum and minimum volume of left atrium showed no statistically significant difference compared with that within 1 week before surgery (all P > 0.05). There was no statistically significant difference between the maximum and minimum volume of left atrium at 5 months after surery and that within 1 week before operation and 48 h after surery (P > 0.05). The difference between the ridge width of left pulmonary vein and left atrial appendage was statistically significant (P < 0.05). There was no significant difference in the interauricular ridge width between left pulmonary vein and left atrial appendage between 5 months and 1 week before surery (P > 0.05); 5 months after surery, the left pulmonary vein and left interauricular crest width were compared with 48 h after surgery, and the differences were statistically significant (P < 0.05). Conclusion There is no increase in pulmonary vein orifice, left atrial volume, left pulmonary vein and left interauricular crest width in patients with recurrent atrial fibrillation after pulmonary vein isolation. Transesophageal echocardiography can provide valuable information for clinical ablation of atrial fibrillation.

95 Application effect of WeChat push in patients with remittent stage of severe depression

Objective To study the application effect of WeChat push in patients with remittent stage of severe depression. Methods A total of 90 patients with severe depression admitted to Department of Psychiatry, Liaocheng Fourth People′s Hospital in Shandong Province from December 2015 to August 2017 were selected as research object, they were divided into the control group and the observation group by random double-blind method, with 45 cases in each group. The control group were treated with conventional therapy, and the observation group regularly promoted WeChat knowledge through WeChat ID on the basis of conventional therapy in the control group. The Hamilton depression scale (HAMD), activity of daily living scale (ADL), Hamilton anxiety scale (HAMA) of the two groups before and after treatment were compared, as well as medication compliance score, recurrence rate of depression and rehospitalization rate of the two groups after treatment were compared. Results Finally, 35 cases were included in the observation group and 39 cases were included in the control group. Before treatment, the HAMD, ADL, HAMA of the two groups were compared, and the differences were not statistically significant (P > 0.05). After treatment, the HAMD, ADL and HAMA scores in the two groups were lower than those before treatment, and the HAMD, ADL and HAMA scores in the observation group were lower than those in the control group, the differences were statistically significant (P < 0.05). The medication compliance score in the observation group was higher than that in the control group, the difference was statistically significant (P < 0.05). After treatment, there was no statistically significant difference in recurrence rate of depression and rehospitalization rate between the two groups (P > 0.05). Conclusion WeChat push can relieve depressive symptoms and anxiety symptoms of patients with major depression, improve medication compliance and activity of daily living of patients.

99 Effect of preinjection of Nalbuphine on Etomidate - induced myoclonus during induction of general anesthesia

Objective To evaluate the effect of preinjection of Nalbuphine on Etomidate - induced myoclonus during induction of general anesthesia. Methods Sixty-four adult patients selected from the First Affiliated Hospital of He′nan University for Urological Surgery under general anesthesia from June to September 2018 were selected. The American society of anesthesiologists class Ⅰ -Ⅱ (ASA), age range from 30 to 63. Random number table method was used to divide into Nalbuphine group (N group) and control group (C group), 32 cases of each group. Before anesthesia induction, N group received 0.2 mg/kg Nalbuphine 10 mL, C group received an equal amount of normal saline, and after 150 s, both groups received 0.3 mg/kg Etomidate. The mean arterial pressure (MAP), heart rate (HR) and oxygen saturation (SpO2) of the two groups were recorded immediately before pretreatment (T0), 1 min after pretreatment (T1), immediately before Etomidate administration (T2) and 2 min after Etomidate administration (T3). Myoclonic condition within 2 min after Etomidate administration was recorded and its severity was assessed. Adverse reactions such as nausea and vomiting bradycardia and hypotension before Etomidate administration were recorded. Results Only 3 cases (9.4%) of mild myoclonus in N group, while in group C, 23 cases (71.9%) had myoclonus of varying degrees, among them, 14 cases (43.8%) were mild, 8 cases (25%) of moderate and 1 case (3.1%) of severe. The incidence of myoclonus in N group was significantly lower than that in C group (P < 0.01). Comparison of MAP, HR and SpO2 at different time points in the two groups showed no statistically significant difference (P > 0.05). During the induction of general anesthesia, the incidence of adverse reactions such as nausea, vomiting, bradycardia and hypotension in the two groups showed no statistically significant difference (all P > 0.05). Conclusion Preinjection of 0.2 mg/kg of Nalbuphine 150 s before intravenous Etomidate injection can effectively inhibit the development of Etomidate induced myoclonus during the induction of general anesthesia.

To investigate the efficacy and safety of CT-guided sphenopalatine ganglion plasma radiofrequency ablation for the treatment of sphenopalatine neuralgia (SPN). Methods From January 2015 to August 2016, 61 patients with SPN treated with CT-guided sphenopalatine ganglion low temperature plasma radiofrequency ablation in the Department of Pain Management of Xuanwu Hospital, Capital Medical University were selected, the VAS scores of patients were compared at before and 1 d, 3 d, 1 week, 1 month, 3 months, 6 months, 1 year and 2 years after the surgery. The MacNab scores were observed at 1 week and 1 year after the surgery. The postoperative facial numbness and complications of patients were also recorded. Results The VAS scores of patients at each time point after surgery were significantly lower than those before surgery (P < 0.05). There was no significant difference in MacNab scores 1 week and 1 year after surgery (P > 0.05), and the excellent and good rates 1 week and 1 year after surgery were 91.8% and 83.6% respectively. The incidence of mild postoperative facial numbness was 31.1% at 1 week after the surgery, there was no postoperative facial numbness patients 1 year after the surgery. All patients had no serious surgical complication. Conclusion Patients with SPN can obtain satisfactory clinical results and mild adverse reactions after the sphenopalatine ganglion plasma radiofrequency ablation, it is worthy of clinical promotion.

107 Efficacy and safety of low temperature plasma ablation cervical dorsal rami nerve in the treatment of cervicogenic headache

Objective To investigate the clinical effect of low temperature plasma ablation on cervical headache (CEH). Methods A total of 40 patients with cervical headache who were admitted to the Pain Management Department of Xuanwu Hospital, Capital Medical University from July 2017 to January 2018 were selected, low temperature plasma ablation of posterior cervical nerve was performed. Visual analogue scale (VAS) and clinical efficacy were used before and after treatment, the pain scores before and after treatment and clinical effects were compared, and the adverse reactions were observed. Results VAS scores at 1 day, 1 week, 1 month and 3 months after operation were lower than those before operation (all P < 0.05), the total effective rate were 92.5%, 95.0%, 95.0% and 87.5% at 1 day, 1 week, 1 month and 3 months after the operation, respectively, there was no statistically significant difference in the total effective rate at all time points (P > 0.05). Postoperative numbness occurred in 6 cases, tinnitus in 1 case, and no serious adverse reactions were found in the others. Conclusion Low temperature plasma ablation of the posterior branch of cervical nerve is an effective method to treat cervical headache with significant effect and no serious adverse reactions.

With the increase of the number of postmenopausal women in recent years, the prevention and treatment of postmenopausal osteoporosis (PMOP) has been paid much attention by the society and has become a health care topic that many countries attach great importance to. From the perspective of traditional Chinese medicine, Zhang Jingyue believes that the yuan yin and yuan yang stored in the vital gate can regulate and balance the yin and yang of the whole body, providing inexhaustible driving force for the normal development of the functions of the five zang viscera and six fu viscera. If the balance of yin and yang is broken, the pathological state of “water loses its source, fire loses its source” will appear. Based on Zhang Jingyue′s academic ideas of “vital gate theory” and “yin and yang interacting with each other”, this study reviews the clinical treatment methods of “yin and yang interacting with each other” of PMOP, so as to provide theoretical basis and new treatment ideas for the prevention and treatment of PMOP.

Objective To explore the characteristics of prescription drugs of curing wind pathogen-weak foot men of Taiping Huimin Heji Ju Fang (volume one). Methods The database was established by using excel 2010 data platform. The commonly used drugs, common couplet medicines, nature and flavour, channel entry were statistically analyzed respectively, and the ranking was conducted according to the frequency of usage, and the corresponding frequency or composition ratio of use was also calculated. Results The volume one, curing wind pathogen-weak foot men contains 87 prescriptions in fact. The commonly used drugs are superficies-relieving formula and deficiency-invigorating formula, accounted for 15.98%, 13.79% respectively; the common couplet medicines are Radix Saposhnikoviae and Rhizoma Ligustici Chuanxiong, Radix Saposhnikoviae and Rhizoma et Radix Notopterygii, both accounted for 16.09%; the drug nature is mainly given priority to warm and hot medicine, accounted for 66.21%; the drug flavour is given priority to pungent medicine, accounted for 41.60%; the channel entry of recorded drugs is mainly spleen and stomach channel, accounted for 28.12%. Conclusion The drugs recorded in volume one are mainly used to dispel wind pathogens, among which, the most frequently used drugs are superficies-relieving formula. The compatibility of drugs is mainly about Radix Saposhnikoviae, the drug nature is mainly given priority to warm and hot, the drug flavor is mainly pungent, and the channel entry is mainly spleen and stomach.

118 Clinical effect of self-made Wenwei Sanhan Decoction in the treatment of chronic atrophic gastritis with spleen-stomach deficiency and cold type

Objective To explore clinical effect of self-made Wenwei Sanhan Decoction in the treatment of chronic atrophic gastritis (CAG) with spleen-stomach deficiency and cold type. Methods One hundred and three CAG patients with spleen-stomach deficiency and cold type admitted to Qinghai Transportation Hospital from March 2016 to August 2018 were selected, they were divided into control group (n = 51) and study group (n = 52) according to the random number table method. Control group was treated with Qiwei Weitong Capsules and study group was treated with Wenwei Sanhan Decoction on the basis of control group. Clinical effect, clinical symptom score and thromboxane B2 (TXB2), pepsinogen (PG) were compared between two groups. Incidence of adverse reactions during the treatment in two groups was observed. Results After treatment of 8 weeks, total effective rate of study group was significantly higher than that of control group, there was a significant difference (P < 0.05). After treatment of 8 weeks, scores of clinical symptoms such as dry mouth and bitter mouth, fullness of upper abdomen, fatigue, poor appetite, dyspepsia in both groups were lower than those before treatment, and those in study group were lower than those in control group (P < 0.05). After treatment of 8 weeks, plasma TXB2 level of two groups was lower than that before treatment, and level of TXB2 in study group was lower than that in control group (P < 0.05). After treatment of 8 weeks, level of plasma PGⅠ in two groups was higher than that before treatment, and that in study group was higher than that in control group (P < 0.05). There was no significant difference in plasma PGⅡ level between two groups before treatment and after treatment of 8 weeks (P > 0.05). There was no significant difference in incidence rate of adverse reaction between two groups during treatment (P > 0.05). Conclusion Self-made Wenwei Sanhan Decoction is effective in the treatment of CAG with spleen-stomach deficiency and cold type. It can effectively improve clinical symptoms of patients and the levels of TXB2 and PG in serum without increasing the incidence rate of adverse reaction. It has high clinical value.

The author believes that the root of lingering and refractory plaque psoriasis is blood stasis, blood-activating method can achieve good effect each time in clinic. Based on “determination of etiologic factors based on differentiation” “recognition of abnormal state based on perception of the objective world” “syndrome test by prescription” and other diagnosis principle, this paper systematically discusses that plaque psoriasis should be “treated from the perspective of stasis” according to the chapter and verse. Objective To explore the clinical efficacy of Qushi Jianfa Decoction combined with plum-blossom needle in the treatment of alopecia seborrheica with syndrome of dampness stagnancy due to spleen deficiency. Methods Ninety patients with seborrheic alopecia admitted to Shunyi Hospital of Beijing Traditional Chinese Medicine Hospital from July 2016 to January 2018 were divided into control group (45 cases) and research group (45 cases) by random number table method. The patients in the control group were treated with conventional treatment, while the patients in the research group were treated with Qushi Jianfa Decoction combined with plum-blossom needle puncture on the basis of the control group. The two groups were treated continuously for 6 months. The clinical total effective rate, the Chinese medicine syndrome scores before and after treatment, the element levels of iron (Fe), calcium (Ca), zine (Zn) and copper (Cu) in the hair and recurrence rate of two groups were compared. Results The total effective rate of research group was significantly higher than that of the control group, the difference was statistically significant (P < 0.05). After treatment, the Chinese medicine syndrome scores of pruritus, greasy, hair loss and scaly in the research group were significantly lower than those in the control group, and the scores of the two groups were significantly lower than those before treatment, the differences were statistically significantly (P < 0.05 or P < 0.01). After treatment, the levels of Fe, Ca, Zn and Cu in the hair in the research group were significantly higher than those in the control group, and the levels of elements after treatment in the two groups were significantly higher than those before treatment, the differences were statistically significant (P < 0.05 or P < 0.01). During the follow-up period, the recurrence rate of the research group was significantly lower than that of the control group, the difference was statistically significant (P < 0.05). Conclusion The clinical curative effect of Qushi Jianfa Decoction combined with plum-blossom needle in the treatment of alopecia seborrhoeica with syndrome of dampness stagnancy due to spleen deficiency is significant, which can significantly improve symptoms, increase the level of trace elements in hair, and has a low recurrence rate.

Based on understanding the original meaning of "thought", discussions by doctors of all dynasties and clinical practice observation, "the emotion of the spleen being anxiety" is not limited to participate in thinking, but also can regulate sadness and sad moods. "Anxiety impairs the spleen" has internal and external causes. Negative emotional stimuli such as over-thinking and sadness, as well as the inherent character and innate endowment of thoughtfulness and sentiment, can affect the function of the spleen. Symptoms caused by "anxiety impairs the spleen" are similar to those caused by depression. Combined with the characteristics of spleen are static and prohe to depression and the pivotal function of spiritual interaction, this paper expounds the basis of treating depression from the spleen. 138 Professor Yan Shuxun′s experience in treating thyroid cancer postoperatively from the liver

Thyroid cancer (TC) is a common malignancy tumor of the endocrine system. Professor Yan Shuxun believes that thyroid diseases, including TC and postoperative TC diseases, are closely related to the liver, whether they are from the meridian circulation or physiological and pathological changes. Professor Yan believes that the common causes of postoperative TC include seven-pathogenic diseases, surgical trauma, iodine and thyroid hormones. He also attaches importance to the differences in the emotional factors of liver qi, liver fire, and liver yang in the process of disease evolution, and proposes that stagnation of liver qi, exuberance of liver fire, and upper hyperactivity of liver yang are the main pathogenesis of early patients in TC and postoperative TC. This article analyzes the disease by summarizing Professor Yan Shuxun′s experience in the treatment of TC from the treatment of liver diseases and combined with Chinese medicine.

142 Value of MRI DWI combined with PI-RADS in screening prostate cancer

Objective To discuss the apparent diffusion coefficient (ADC) value of 1.5T magnetic resonance (MRI) combined with PI-RADS score in screening prostate cancer (PCa). Methods Forty patients with PCa were selected from December 2014 to September 2018 in the Second Affiliated Hospital of Bengbu Medical College, and as PCa group. At the same time, 60 patients with benign prostate were selected as benign group. The two groups were given 1.5T-MRI examination. The prostate imaging ADC value, eADC value and PI-RADS score of the two groups were compared, the diagnosis performance of PCa with different methods were compared. Results The PI-RADS score and eADC value in PCa group were higher than those in the benign group, while ADC value was lower than that in the benign group, the differences were highly statistically significant (P < 0.01). The ROC curve analysis showed that, the sensitivity, specificity and Youden index of ADC value combined with PI-RADS score in the diagnosis of PCa were significantly higher than the ADC and PI-RADS score alone, and the area under the curve of combined diagnosis was the biggest. Conclusion The ADC value of 1.5T-MRI combined with PI-RADS score can be used as a primary indicator of PCa screening. And it can effectively improve the screening effect of PCa, it is worth for clinical promotion and application.

146 Evaluation of morphological changes of the levator hiatus under different conditions in elderly women with stress urinary incontinence base on the three-dimensional ultrasound

Objective To evaluate the morphological changes of the levator hiatus (LH) under different conditions in elderly women with stress urinary incontinence (SUI) base on three-dimensional ultrasound. Methods Thirty elderly female SUI patients from June 2017 to May 2018 in Dayi Hospital Affiliated to Shanxi Medical University ("our hospital" for short) were selected as the case group, while 30 cases taken physical examination in our hospital and excluded from SUI were selected as control group. Three dimensional reconstruction technology was used to measure the morphological changes of the pelvic diaphragm in three states of resting, maximum voluntary contraction (MVC) and Valsalva, including the characteristic parameters of anterior and posterior diameter (LHap), transverse diameter (LHt), circumference (LHL), area (LHarea), as well as the pubic visceral muscle thickness. Results At the resting state, LHt and LHarea in the case group were significantly increased compared with the control group (P < 0.05). In MVC state, LHaP, LHt, LHL and LHarea were increased in the case group compared with the control group, and the differences in LHaP and LHt between two groups were statistically significant (P < 0.05). Under the condition of Valsalva, LHt, LHL, LHarea, and thickness of pubic visceral muscle increased in the case group compared with the control group(P < 0.05), while LHap had no statistical significance (P > 0.05). Conclusion Based on three-dimensional ultrasound, significant differences can be observed in morphological measurements of the levator hiatus in elderly women with SUI in different states, which is helpful for the diagnosis and evaluation of SUI in elderly women.

149 Effect of Valsartan Capsules combined with Amiodarone Hydrochloride in the treatment of dilated cardiomyopathy with ventricular arrhythmia

Objective To investigate the clinical effect of Valsartan Capsules combined with Amiodarone Hydrochloride in the treatment of dilated cardiomyopathy (DCM) with ventricular arrhythmia (VA). Methods Eighty DCM patients with VA admitted to Danzhou People′s Hospital from April 2017 to June 2018 were selected. The patients were divided into the control group (n = 40) and the study group (n = 40) according to the random number table method. The control group was treated with Aamiodarone Hydrochloride, and the study group was treated with Valsartan Capsules on this basis, both of which were treated continuously for 3 months. The clinical effect of the two groups after treatment, the left ventricular remodeling index [left ventricular ejection fraction (LVEF), left ventricular end-diastolic diameter (LVEDD), left ventricular end systolic diameter (LVESD), N terminal b-type brain natriuretic peptide (NT - proBNP)], QT dispersion degree index [QT dispersion degree (QTd), corrected QT dispersion degree (QTcd)] between two groups before and after the treatment were compared, the adverse reactions of two groups during treatment were recorded. Results The total effective rate of the study group was higher than of the control group (P < 0.05). After treatment, LVEF of patients in the two groups was increased compared with that before treatment, and the study group was higher than that of the control group (P < 0.05). LVEDD, LVESD and NT-proBNP were all lower than those before treatment, and the study group was lower than the control group (P < 0.05). After treatment, QTd and QTcd of both groups were lower than those before treatment, and the research group was lower than the control group (P < 0.05). Comparison of the total incidence of adverse reactions between the two groups showed no statistically significant difference (P > 0.05). Conclusion Valsartan Capsules combined with Amiodarone Hydrochloride for the treatment of DCM patients with VA has a significant effect, which can effectively improve the left ventricular remodeling and QT dispersion in patients without increasing the incidence of adverse reactions, which has a certain clinical application value.

153 The transbronchial needle aspiration combined with rapid on-site evaluation in diagnosis of 18 cases with mediastinal lesions and literature review

Objective To evaluate the diagnostic value of transbronchial needle aspiration (TBNA) combined with rapid on-site evaluation (ROSE) technique in mediastinal lesions. Methods Eighteen patients with mediastinal lesions from June 2016 to June 2018 in the First People′s Hospital of Lianyungang were selected, all the patients were given the detection of TBNA and ROSE technique. The diagnostic efficiency of TBNA combined with ROSE, and diagnose accordance rate with final pathological examination were calculated, the occurrence of complications of TBNA combined with ROSE technique were observed. Results The sensitivity of TBNA combined with ROSE technique in the diagnosis of mediastinal lesions was 70.6%, the specificity was 100.0%, positive predictive value was 100.0%, negative predictive value was 16.7%, miss rate was 29.4%, and no misdiagnosis patient. The diagnose accordance rate of BNA combined with ROSE technique and final pathological examination was 72.2%, the complications included 2 cases of sinus tachycardia, 5 cases of mild hemorrhage, and 1 case of hoarseness, no massive hemorrhage. Conclusion TBNA combined with ROSE technique has high diagnostic efficiency in diagnosing mediastinal lesions, which could significantly reduce repeated TBNA.

158 The predictive value of procalcitonin monitoring in diagnosing the type of bloodstream infection

Objective To explore the predictive value of procalcitonin (PCT) monitoring in diagnosing the type of bloodstream infection. Methods Clinical data of 172 patients with bloodstream infection in Hubei Provincial Hospital of Traditional Chinese Medicine from January to October 2018 were retrospectively analyzed, all patients with positive blood culture and simultaneous detection of blood routine, PCT and C-reaction protein (CRP). According to the results of blood culture, patients were divided into gram-positive bacteria (G+) bacteria infection group (55 cases) and gram-negative bacteria (G-) bacteria infection group (117 cases). The leves of white blood cell count, neutrophil count neutrophil ratio, neutrophil lymphocyte ratio, CRP and PCT were compared between two groups. The ROC curve was used to evaluated the diagnostic value. Results Patients of the G+ bacteria infection group were mainly from the department of emergency and infectious disease, the main infection bacterium was Staphylococcus. Patients of the G- bacteria infection group were mainly from the department of infectious disease and hematology, the maininfection bacteria was Escherichia coli. The PCT level of patients in G- bacteria infection group was higher than that of patients in G+ bacteria infection group, and the difference was statistically significant (P < 0.05). When 1.11 ng/mL was as the optimal cut-off point value, the sensitivity of PCT was 55.08% and the specificity was 80.00%. Conclusion PCT has a good predictive value for the diagnosis of bloodstream infection types, and the detection of different bacterial infection types is significantly different.

166 Application value of PDCA circulation in the management of specialized antibacterial drugs in our hospital

Objective To evaluate the application value of PDCA circulation in the management of specialized antibacterial drugs in Pidu District People′s Hospital of Chengdu City ("our hospital" for short), in order to reduce irrational expenditure on medical insurance, and promote rational clinical use of drugs. Methods PDCA circulation was adopted to manage the use of specialized antibacterial drugs from January to December 2018 in our hospital, which was compared with the time when the PDCA circulation management had not been applicated (from January to December 2017). Related data was extracted from the hospital information system. The sales amount, the rate of pre-use specimen submission, the number of patients, medication days, defined daily dose system (DDDs), defined daily cost (DDC) and drug utilization index (DUI) were compared before and after the application of PDCA circulation management. Results After PDCA circulation management, the sales amount of specialized antibacterial drugs in 2018 dropped by 35.86% compared with that in 2017, among which the biggest decline was 75.39% for Aztreonam. Overall DDDs decreased by 11.21%, DUI was almost lower than 1 except for Norvancomycin Hydrochloride. The number of patients who used these drugs and medication days had a slight decrease. The rate of pre-use specimen submission increased from 91.03% to 97.96%. Conclusion The PDCA circulation management of specialized antibacterial drugs can promote rational drug use and decrease medical insurance expense, which has the value for expansion and application.

171 Analysis on control effect of medical expenses in public hospitals in shandong Province

Objective To explore the effect of medical cost control in public hospitals in Shandong Province, and to provide reference and suggestions for medical cost control in public hospitals. Methods The data related to outpatient and inpatient medical expenses of public hospitals in Shandong Province from 2010 to 2017 were statistically analyzed and the changes of medical expenses were analyzed. Results The average outpatient medical expenses of Shandong public hospitals increased from 167.3 yuan in 2010 to 249.3 yuan in 2017, with an average annual growth rate of 5.9%. Per capita medical and medical expenses per resident increased from 6415.9 yuan in 2010 to 9628.1 yuan, an average annual growth rate of 6.0%. From 2010 to 2017, the growth rate of outpatient and inpatient medical expenses in public hospitals in Shandong Province was reasonable. The proportion of outpatient and inpatient drugs decreased year by year, while the proportion of examination, treatment and operation expenses increased. In all levels of hospital 3 class hospital medical treatment cost is the highest, one class hospital is the lowest. Conclusion The policies and measures of medical expense control in public hospitals in Shandong Province have some effects, but they are not significant enough. It is suggested that the government actively promote relevant policy reform, increase investment and accelerate the construction of hierarchical diagnosis and treatment system. We will accelerate reform of the pharmaceutical market, establish an "economy-equity" evaluation system for benefit indicators, and effectively control medical expenses.

175 Role of cluster intervention in the prevention of deep vein thrombosis in patients undergoing laparoscopic surgery for intestinal diseases

Objective To evaluate the role of cluster intervention in the prevention of deep vein thrombosis in patients undergoing laparoscopic surgery for intestinal diseases. Methods From April 2017 to May 2018, 267 patients with intestinal diseases underwent laparoscopic surgery at Department of General Surgery, the First Affiliated Hospital of Xi′an Jiaotong University were divided into two groups by random number table method. Observation group (133 cases) was treated with cluster intervention nursing, and control group (134 cases) was treated with routine nursing in general surgery. The coagulation function, first exhaust time after operation, hospitalization time, hospitalization expenses, complications and satisfaction of nursing quality evaluation were compared between two groups. Results After operation of 5 days, the levels of fibrinogen and D-dimer in both groups were higher than those before operation, but those in observation group were lower than those in control group, with statistical significance (all P < 0.05). The first exhaust time after operation and hospitalization time of observation group were significantly shorter than those of control group, and the hospitalization expenses of observation group were less than those of control group, the differences were statistically significant (P < 0.05 or P < 0.01). The incidence of postoperative complication in observation group was lower than that in control group (P < 0.05). When discharged from hospital, total satisfaction of nursing quality evaluation in observation group was higher than that in control group, and the difference was statistically significant (P < 0.05). Conclusion Cluster intervention can significantly improve the coagulation status of patients undergoing laparoscopic surgery of intestinal disease, effectively reduce the incidence of deep vein thrombosis, significantly shorten hospitalization time and reduce medical expenses of patients. It is conducive to improving the quality of life of patients and is worthy of reference by the clinical nursing team.

179 Study on the construction of the indicator system of obstetric nursing safety management in Beijing based on Delphi method

Objective To explore and construct indicator system of Beijing′s obstetric nursing safety management, and provide content basis and quantitative standards for obstetric nursing safety management. Methods From December 2018 to June 2019, literature review and brainstorming were used by research team to draw up a quantitative table indicator pool. Then 20 nursing specialists were selected from the First Hospital of Beijing University, Peking University People′s Hospital, International Hospital of Beijing University, Beijing Obstetrics and Gynecology Hospital, Beijing Air Force General Hospital, Beijing Yanhua Hospital, Beijing Century Temple Hospital, Xuanwu Hospital, Capital Medical University, 304 Hospital of PLA and Beijing Luhe Hospital, Capital Medical University (10 hospitals). Delphi method was used by nursing specialists to score the scientificity and operability of the primary indicators in two rounds. The indicator system of obstetric nursing safety management in Beijing was constructed by screening evaluation indicator with boundary value method and calculating indicator weight with percentage weight method. Results The recovery rates of two rounds of inquiry papers were 100%, and the authoritative coefficients of experts were more than 0.70; experts had consistency of Kendall′s W coordination test (P < 0.01). Through the first round of expert consultation: delete 1 secondary indicator, 6 tertiary indicators and increase 1 tertiary indicator; the second round of expert consultation: delete 2 tertiary indicators. This study constructed the index system of obstetric nursing safety management in Beijing: 3 first-level indicators, 11 second-level indicators and 33 third-level indicators. Conclusion The indicator system of obstetric nursing safety management in Beijing is scientific and reliable, which can provide a basis for the evaluation of obstetric nursing safety in Beijing.

185 Application value of hierarchical chain model in pediatric pneumonia

Objective To explore application value of hierarchical chain model in pediatric pneumonia. Methods Clinical data of 120 children with pneumonia in Baoji Maternal and Child Health Hospital of Shaanxi Province from January 2013 to December 2018 was retrospectively analyzed. According to the different intervention measures, they were divided into two groups: control group (routine nursing intervention, 60 cases) and observation group (hierarchical chain model, 60 cases). The disappearance time of fever, shortness of breath, cough, lung rale, antibiotic use time, atomization treatment time and hospitalization time of two groups were observed. The degree of cooperation and satisfaction of the families of two groups were observed, and clinical efficacy of two groups was observed. Results The disappearance times of fever, shortness of breath, cough, lung rale, antibiotic use time, atomization treatment time and hospitalization time in observation group were shorter than those in control group. The degree of cooperation and satisfaction of family members in observation group were higher than those in control group. Total clinical effective rate in observation group was higher than that in control group, and the differences were statistically significant (P < 0.05). Conclusion Application of hierarchical chain model in pediatric pneumonia shows that the disappearance time of clinical symptoms of children is obviously shortened, and the degree of cooperation and satisfaction of family members of children with pneumonia are improved. The effect is remarkable, which is worthy of clinical popularization and application.
