Research on the effects of ligustrazine phosphate on angiotensin Ⅱ induced hypertrophy of H9c2 cells and its mechanism

Objective To investigate the effects of ligustrazine phosphate (ligu) on hypertrophy of H9c2 cells induced by angiotensin Ⅱ (Ang Ⅱ) and its possible mechanism. Methods H9c2 cells hypertrophy induced by Ang Ⅱ was used as a model. The cells were randomly divided into control group, model group, ligustrazine phosphate group, ligustrazine phosphate + NE-100 group and NE-100 group. Rhodamine-Phalloidin staining was used to observe the changes of cell area; Fura-2 AM was used to determine the changes of intracellular Ca2+ concentration; Western blot was used to detect the expression of Sigma-1 receptor (Sig-1R), calcineurin (CaN) and inositol 1, 4, 5-trisphosphate receptor (IP3R). Results Ligustrazine phosphate could inhibit the increase of cell area induced by Ang Ⅱ (P < 0.05), which could be blocked by NE-100, a Sig-1R inhibitor (P < 0.05), and Ligustrazine phosphate could inhibit the increase of BNP mRNA level induced by Ang Ⅱ (P < 0.05). Ang Ⅱ incubation decreased the expression level of Sig-1R protein (P < 0.05), ligustrazine phosphate had a tendency to restore the expression level of Sig-1R protein (P > 0.05); ligustrazine phosphate intervention could significantly reduce the increase of IP3R protein level induced by Ang II (P < 0.05), NE-100 blocked the effect (P < 0.05); but ligustrazine phosphate intervention had no effect on the increase of CaN induced by Ang Ⅱ (P > 0.05). In addition, ligustrazine phosphate inhibited the increase of intracellular Ca2+ in H9c2 cells induced by Ang Ⅱ, which was blocked by NE-100 (P < 0.05). Conclusion Ligustrazine phosphate inhibits cardiomyocyte hypertrophy by interfering with the expression of IP3R through Sig-1R and affecting intracellular calcium balance.

Effects of pepper acid ethylenediamine on blood lipid levels and antioxidant capacity in hyperlipidemia rats

Objective To study the effects of pepper acid ethylenediamine (PAE) on blood lipid levels and antioxidant capacity in hyperlipidemia rats. Methods Sixty-five SPF male Sprague-Dawley rats were selected. Their basic serum cholesterol (TC) and triglyceride (TG) were determined. 10 rats were selected as the normal control group and given normal diets. 55 rats were fed with high-fat diet as model groups. After 4 weeks of intervention, TC and TG were detected again. Compared with the blood lipid levels before and after modeling, TC and TG were significantly increased, then the model was believed to have been built successfully. The 50 rats with successful modeling were divided into 5 groups: high-fat model group (n = 10), positive drug group (Simvastatin, 10 mg/kg, n = 10), and high (10 mg/kg, n = 10), medium (5 mg/kg, n = 10) and low (2.5 mg/kg, n = 10) dose of PAE groups. Each group was continuously intragastrically administered for 4 weeks, once a day. At the end of the fourth week, serum was collected for analysis of the levels of TC, TG, high density lipoprotein (HDL-C), low density lipoprotein (LDL-C), total superoxide dismutase (T-SOD), glutathione peroxidase (GSH-Px) and malondialdehyde (MDA). Results Compared with the normal control group and hyperlipidemia model group, each dose of PAE groups could reduce the levels of TC, TG, LDL-C and raise HDL-C (P < 0.05 or P < 0.01); compared with the positive drug group, the levels of LDL-C in high dose of PAE group were decreased (P < 0.05). Compared with the high fat model group, the activity of T-SOD and GSH-Px in high dose of PAE group increased (P < 0.05 or P < 0.01), the content of MDA in positive drug group and middle dose of PAE group was significantly decreased (P < 0.01). Compared with the positive drug group, the content of MDA in high dose of PAE group inecreased (P < 0.05). Conclusion PAE can reduce blood lipid levels, increase the activity of serum T-SOD, GSH-Px, and decrease MDA content in hyperlipidemia rats, suggesting that the hypolipidemic effect of PAE may be ralated to improving oxidative stress in rats with high fat diet.

The effects of Lycium barbarum and Salvia miltiorrhiza on the histomorphology and CRYAB mRNA of retinal pigment degeneration rats

Objective To observe the effects of Lycium barbarum and Salvia miltiorrhiza on the morphology of retinal tissue and alpha B-crystalline (CRYAB) mRNA in rats with retinitis pigmentosa. Methods Twenty-four male and female retinitis pigmentosa RCS (rdy-/-, p-/-) rats were divided into a model group and a ginseng group according to random number table method, with 8 rats in each group, 4 males and 4 females. Another 8 RCS (rdy+/+, p+/+) rats, 4 males and 4 females, were blank group. Oral gavage was started 1 week after routine feeding, once a day. The blank group and the model group were given normal saline, and the ginseng group was given an equivalent dose of Lycium barbarum + Salvia miltiorrhiza. The rats were sacrificed for 30 days after continuous gavage. The morphological changes of the outer nuclear layer of the retina of each group were observed under light microscope. The expression of CRYAB mRNA in RCS rats was detected by qPCR. Results The results of HE staining showed that the retinal structure of each layer in the blank group was neatly arranged and regular in shape. The retinal structure of the model group was looser, with degeneration and atrophy, and the thickness of the outer nuclear layer was significantly thinner, the difference was highly statistically significant (P < 0.01). Compared with the blank group, the ginseng group had a disordered structure and thinner thickness, but it had increased thickness compared with the model group, and the difference was highly statistically significant (P < 0.01). The results of qPCR showed that the expression of CRYAB mRNA was observed in the retina of rats in the blank group. The expression of CRYAB mRNA in the retina of the model group was significantly decreased compared with the blank group (P < 0.01). The expression of CRYAB mRNA in the retina of the ginseng group was higher than that in the model group and the difference was highly statistically significant (P < 0.01). Conclusion RCS rats can well simulate the pathogenesis of human retinitis pigmentosa. The structure of the retina is disordered, the outer nuclear layer is thinned, the expression of CRYAB mRNA in the retina is inhibited, and Lycium barbarum + Salvia miltiorrhiza can induce the expression of CRYAB mRNA in the retina. It has a certain protective effect on the retina of retinitis pigmentosa RCS rats.

Effect of Genistein on FSHR gene expression in ovarian granulosa cells of female rats

Objective To observe the effect of Genistein on the expression of follicle stimulating hormone receptor (FSHR) gene in ovarian granulosa cells of female rats and its relationship with estrogen receptor (ER). Methods Female rats aged 25-28 days were subcutaneously injected with pregnant mare serum gonadotrophin to establish follicular development model. Ovarian granulosa cells were isolated and cultured. Genistein (0, 0.1, 1,5, 10, 100 μmol/L) and estrogen receptor blocker ICI 182,780 (1 μmol/L) were treated for 24 hours respectively. Reverse transcription-polymerase chain reaction (RT-PCR) was used to detect the expression of FSHR mRNA, an upstream regulator of aromatase gene. Results Compared with the blank control group, the expression of FSHR mRNA was significantly increased in the 10 μmol/L Genistein group (P < 0.05). After ICI 182,780 (1 μmol/L) pretreatment for 30 minutes, the expression of FSHR in ICI 182,780 pretreated cells with 1 and 10 μmol/L Genistein was significantly lower than that in the 1 and 10 μmol/L Genistein groups (P < 0.05 or P < 0.01) respectively. Conclusion 10 μmol/L of Genistein can up-regulate the expression of FSHR in ovarian granulosa cells of female rats, and this effect may be mediated by ER.

Advances of cerebrospinal fluid enzyme spectrum in Alzheimer′s disease

Alzheimer′s disease (AD) is an age-related and progressive neurodegenerative disease, which is the most common disease leading to dementia. AD develops slowly and becomes severe progressively. In the development of AD, different stages are accompanied with corresponding pathophysiological changes, where the enzymes in the cerebrospinal fluid of patients with AD can be used as biomarkers to reflect the developing process of AD, and before the dementia stage of AD, there is a preclinical stage ranging from years to decades. In this way, it is of great significance to the early diagnosis and treatment of AD. This review focuses on the current research of some enzymes in the cerebrospinal fluid of AD.

Advances in immune regulation of TLR 2/4-mediated inflammatory response by IDO in infection status

In control or treatment of infection diseases, immune tolerance can affect the efficacy of immunotherapy. At the same time, indoleamine 2, 3-dioxygenase (IDO)-mediated immune tolerance and TLR2/4-initiated immune response often co-exist. IDO inhibits T cell proliferation or promotes T cell apoptosis by mediating tryptophan metabolism and promoting high expression of regulatory T cells(Tregs). At the same time, increased Tregs can promote the expression of IDO by binding to the receptors of DCs. At present, IDO inhibitors have been proved to play an important role in cancer treatment. This article reviews the pathways of TLR2/4 and IDO, which provides some ideas for future targeted therapy.

Advance research on the role of cancer-associated fibroblasts in oral squamous cell carcinoma and its targeted therapy

Oral squamous cell carcinoma (OSCC) is the sixth most common malignant tumor in the world with a five-year survival rate of less than 50%. Its pathogenesis has not been fully elucidated. Cancer-associated fibroblasts (CAFs), as one of the important cellular components in the tumor microenvironment, play an important role in tumor invasion, metastasis and angiogenesis. CAFs promote tumor development mainly by releasing growth factors, modifying extracellular matrix, supporting angiogenesis, and inhibiting anti-tumor immune responses. CAFs-targeted therapy may effectively inhibit the invasion and metastasis of OSCC, and limit tumor angiogenesis, which provides new strategies for the treatment of OSCC. This article comprehensively discusses the advance research on the role of CAFs in OSCC and its targeted therapy.

Research progress of the pre-hospital assessment scale for acute ischemic stroke

Acute stroke is a kind of medical emergency that seriously endangers human health. It has the characteristics of high incidence, high mortality, high recurrence rate, high disability rate and high cost. If stroke patients get treatment earlier, the more benefits they get. Pre-hospital evaluation scale of stroke can assist medical staff to identify acute stroke. The sensitivity and specificity of different pre-hospital evaluation scales of stroke are different, and the reasonable use of pre-hospital evaluation scales of stroke can improve the diagnosis of acute stroke. In the case of no delay in treatment time, the identification of large vessel occlusion should be improved as much as possible, and patients with large vessel occlusion should be immediately sent to the stroke center where endovascular treatment is feasible. Exploring the significance of pre-hospital assessment scale for the construction and treatment of acute stroke treatment system, this article reviews the research progress of the pre-hospital assessment scale for stroke in China and abroad.

Survey of literacy of traditional Chinese medicine of residents in Guangzhou

Objective To explore the status of literacy of Traditional Chinese Medicine of residents in Guangzhou, and to provide scientific basis for formulating relevant traditional Chinese medicine policies and carrying out comprehensive intervention. Methods In the form of face-to-face questionnaires or online questionnaires, the survey was conducted in universities, communities and internet platforms in Guangzhou from January 2016 to January 2017. Results A total of 550 questionnaires were sent out, with an effective recovery of 476 copies. The effective rate of recovery was 86.5%. 84.2% of Guangzhou residents wound take the initiative to learn the health care knowledge of traditional Chinese medicine. 62.4% of the residents would be the first choice of Chinese medicine treatment. 74.2% of Guangzhou residents had a good grasp of the Chinese health care basic theory knowledge. 18.1% of the residents understood the traditional Chinese medicine health care skills and persist in using them. Conclusion Guangzhou residents had obvious demand for health care knowledge of traditional Chinese medicine, but there were still some problems such as one-sided mastery of relevant knowledge and unsatisfactory application of daily health care methods of traditional Chinese medicine. In the future, we should integrate interactive experience elements into health care science popularization activities of traditional Chinese medicine, make full use of the Internet to disseminate correct health care knowledge of traditional Chinese medicine. Popular science propaganda should be professional, comprehensive and authoritative.

Investigation on anxiety status of young adults with pulmonary tuberculosis

Objective To understand and analyze the anxiety status of young and middle-age patients with pulmonary tuberculosis initially treated, and to provide evidence for clinical nursing. Methods A total of 96 newly diagnosed pulmonary tuberculosis patients aged 18-45 years old who were admitted to the Department of Tuberculosis, Beijing Chest Hospital, Capital Medical University, from March 1 to June 30, 2018, were selected as subjects, and through the form of questionnaires, the anxiety status was investigated with the Self-Rating Anxiety Scale (SAS) and the reasons were analyzed. Results The SAS anxiety scores of 96 newly diagnosed hospitalized young and middle-age patients with pulmonary tuberculosis were statistically significantly different from those in China (P < 0.01). The SAS anxiety scores of patients with self-paying was higher than that in patients with public medical care, with statistically significant differences (P < 0.05). Conclusion The anxiety score of young and middle-age patients with pulmonary tuberculosis initially treated is higher than that of the national level. The frequent negative emotions can reduce the immunity and further worsen the disease, thus forming a vicious circle and delaying treatment, which should be paid attention to.

Study on the correlation of plasma suPAR and D-dimer levels with plaque vulnerability in patients with atherosclerosis cerebral infarction

Objective To measure the levels of soluble urokinase plasminogen activator receptor (suPAR) and D-dimer in patients with atherosclerotic cerebral infarction (ACI), and to explore the correlation between them and carotid plaque stability. Methods The clinical data of 90 patients with ACI admitted to Xiangya Hospital of Central South University from January 2017 to January 2018 was retrospectively analyzed and the patients were divided into instability plaque group (42 cases) and unstable plaques group (48 cases) by stability of carotid atherosclerosis. While 50 cases of outpatient health examinations were selected as control group at the same time. The plasma suPAR and D-dimer levels were detected and compared among the three groups. The correlation of plasma suPAR and D-dimer levels with carotid plaque stability was analyzed. Results ①The difference in suPAR and D-dimer expression among three groups was statistically significant (P < 0.01), suPAR and D-dimer levels in the unstable plaque group were significantly higher than those in the stable plaque group and the control group (P < 0.01), and suPAR and D-dimer levels in the plaque stability group were significantly higher than those in the control group (P < 0.01). ②suPAR and D-dimer were risk factors for plaque stability [OR = 1.068, 95%CI (0.002-0.215), P < 0.05; OR = 1.013, 95%CI (0.014-0.462), P < 0.05]. ③The optimal cut-off point for suPAR to predict the unstability of carotid plaques was 8.04 ng/mL. The area under the ROC curve was 0.660 (95%CI: 62.7%-96.5%, P < 0.05). The optimal cut-off point for D-dimer to predict the unstability of carotid plaques was 3.92 ng/mL. The area under the ROC curve was 0.837 (95%CI: 66.1%-99.8%, P < 0.05). Conclusion suPAR and D-dimer can be indicators to evaluate carotid plaque stability in patients with atherosclerotic cerebral infarction.

Meta-analysis of Xuezhikang Capsules in the treatment of patients with dyslipidemia

Objective To evaluate the clinical efficacy and safety of Xuezhikang Capsules in the treatment of patients with dyslipidemia. Methods The databases of PubMed, Science Citation Index-Expanded, China National Knowledge Infrastructure, Wanfang Databases, Chinese VIP Information were searched up to June 2018 for randomized controlled trials about Xuezhikang in the treatment of dyslipidemia. Revman 5.3 software was used to make Meta-analysis. The effects of Xuezhikang Capsules on the serum levels of total cholesterol (TC), low density lipoprotein-cholesterol (LDL-C) and high density lipoprotein-cholesterol (HDL-C) were evaluated. Results A total of 25 trials containing 7875 participants were analyzed. Meta-analysis showed that compared with blank control group, Xuezhikang could lower serum levels of TC (WMD = -1.05, 95%CI: -1.40, -0.70, P < 0.01) and LDL-C (WMD = -0.76, 95%CI: -0.94, -0.58, P < 0.01) more effectively, and increase HDL-C (WMD = 0.17, 95%CI: 0.08, 0.27, P < 0.01) more effectively. Compared with statins control group, Xuezhikang could increase HDL-C (WMD = 0.17, 95%CI: 0.04, 0.30, P < 0.05) more effectively, and also decrease serum TC and LDL-C levels, while there were no significant differences (P > 0.05). Compared with control group, Xuezhikang showed advantages in decreasing serious side effects or major adverse cardiovascular events (WMD = 0.55, 95%CI: 0.47, 0.64, P < 0.01). Conclusion Xuezhikang can regulate dyslipidemia effectively and comprehensively, with low rates of adverse events.

Relationship between Q-T interval dispersion on electrocardiogram and adverse cardiac events in patients with type 2 diabetes mellitus and chronic heart failure

Objective To study the relationship between Q-T interval dispersion (QTd) on electrocardiogram and adverse cardiac events in patients with type 2 diabetes mellitus and chronic heart failure. Methods Two hundred patients with type 2 diabetes mellitus and chronic heart failure admitted to Hengshui Harrison International Peace Hospital in Hebei Province from February 2016 to December 2017 were selected as the subjects. All patients were followed up for 6 months, and they were divided into adverse cardiac events group (n = 63) and non-adverse cardiac events group (n = 137) according to whether the patient had adverse cardiac events. The basic data, the level of biochemical index and echocardiographic index of the two groups were compared, and multivariate Logistic regression analysis was performed. Results The level of uric acid (UA), QTd and corrected QTd (QTcd) in the adverse cardiac events group was higher than that in the non-adverse cardiac events group (P < 0.05). The left ventricular ejection fraction (LVEF) level in the adverse cardiac events group was lower than that in the non-adverse cardiac events group (P < 0.05). The multiple Logistic regression analysis showed that UA, QTd and QTcd were independent risk factors for the occurrence of adverse cardiac events in patients with type 2 diabetes mellitus and chronic heart failure (P < 0.05), while LVEF was a protective factor (P < 0.05). Conclusion The increase of QTd and QTcd in electrocardiogram in patients with type 2 diabetes mellitus and chronic heart failure increases the risk of adverse cardiac events, which is worthy of clinical attention.

A randomized controlled trial on total resection and subtotal resection for supratentorial gliomas associated with epilepsy

Objective To compare the clinical effect of total resection and subtotal resection in the treatment of supratentorial gliomas with epilepsy. Methods A total of 56 patients with supratentorial glioma and epilepsy were admitted to Ya′an People′s Hospital from April 2016 to April 2017, 28 cases in each group. Total tumor resection was performed in the observation group and subtotal tumor resection was performed in the control group. Epilepsy control, Karnofsky performance status (KPS) score, complications were compared between the two groups. Results The satisfaction rate of epilepsy control in the observation group was significantly higher than that in the control group (P < 0.05), and the KPS score in the observation group at 3 months and 1 year after operation were higher than those in the control group (P < 0.05). The complications of the two groups had no statistically significant difference (P > 0.05). Conclusion Total resection for supratentorial glioma with epilepsy has the same effect as subtotal resection, but it can improve the satisfaction rate of epilepsy control, improve the quality of life, reduce the recurrence rate and it has high clinical value.

Study of clinical value of serum TNF-α and IL-1β in neonates with intracranial hemorrhage

Objective To investigate the clinical value of serum TNF-α and IL-1β in neonates with intracranial hemorrhage. Methods From January 2015 to January 2017, 98 neonates with intracranial hemorrhage in Tianmen First People′s Hospital were selected as the subjects (intracranial hemorrhage group), and 50 healthy newborns at the same time were selected as the control group. The levels TNF-α and IL-1β in plasma were detected by ELISA assay at 24 h, 3, 7 d after birth respectively. The patients were followed up for 1 year, and the prognosis was statistically analyzed. Results The levels of plasma TNF-α and IL-1β in intracranial hemorrhage group were significantly higher than those in control group (P < 0.05). At the same time, plasma TNF-α and IL-1β levels gradually decreased after birth (P < 0.05). There was a significant difference in plasma TNF-α and IL-1β in neonates with bleeding volume in 24 h (P < 0.05). The neonates with highest blood volume had the highest plasma TNF-α and IL-1β levels (P < 0.05). There was a positive correlation between the amount of bleeding and the levels of TNF-α and IL-1β in neonates with intracranial hemorrhage (r = 0.409, 0.376; P < 0.05). The plasma TNF-α and IL-1β levels in the cure group were significantly lower than those in the unhealed group (P < 0.05). Conclusion The expression of TNF-α and IL-1β in plasma of neonates with intracranial hemorrhage increased abnormally, and positively correlated with the amount of hematoma. The prognosis of neonates with intracranial hemorrhage is closely related to plasma TNF-α and IL-1β, plasma IL-1β and TNF-α are significantly increased in neonateswith poor prognosis.

Predictive value of peripheral blood fetal hemoglobin, placental growth factor and uterine artery pulsation index for re-pregnancy early preeclampsia in patients with early onset preeclampsia history

Objective To explore the predictive value of combined detection of peripheral blood fetal hemoglobin, placental growth factor, and uterine artery pulse index for re-pregnancy early preeclampsia in patients with early-onset preeclampsia history. Methods From January 2010 to August 2017, in Tangshan Maternity and Child Health Care Hospital, 94 premature pregnancy with preeclampsia history were selected as the subjects. The predictive value of maternal placental growth factor (PLGF), peripheral blood fetal hemoglobin (HbF) and uterine artery pulsatility index (PI) for early onset preeclampsia was observed and analyzed. Results Among 94 pregnant women, 21 cases occurred early onset preeclampsia, the incidence of which was 22.34%. The PLGF level in early onset preeclampsia pregnant women was lower than non early onset preeclampsia pregnant women (P < 0.05), and HbF and PI levels were higher than those in non early onset preeclampsia pregnant women (P < 0.05). The sensitivity of PLGF positive or HbF positive or PI positive in predicting preeclampsia was higher than that of all PLGF, HbF and PI positive (P < 0.05). The specificity of PLGF positive or HbF positive or PI positive in predicting preeclampsia was higher than that of all PLGF, HbF and PI positive (P < 0.05). Conclusion PLGF, HbF and PI are all positive can be taken as prediction criteria to reduce the rate of misdiagnosis, which is beneficial to the diagnosis of early onset preeclampsia. PLGF, HbF or PI positive can be taken as a prediction standard to reduce the missed diagnosis rate, it is beneficial to the elimination of early onset preeclampsia.

Effect of Yangyin Shugan Capsules and Clomiphene Citrate in the treatment of polycystic ovarian syndrome with infertility

Objective To explore the clinical effect of Yangyin Shugan Capsules combined with Clomiphene Citrate in the treatment of polycystic ovary syndrome(PCOS) associated with infertility. Methods A total of 83 PCOS associated with infertility admitted in Zhongshan Hospital of Traditional Chinese Medicine from February 2016 to February 2018 were selected as the research subjects. Patients were divided into control group (n = 41) and experimental group (n = 42) according to the random number table method. The control group was treated with Clomiphene Citrate and the experimental group was treated with Yangyin Shugan Capsules combined with Clomiphene Citrate. The clinical efficacy, ovulation and pregnancy outcome and the incidence of ovarian hyperstimulation syndrome (OHSS) after treatment were compared between the two groups. The levels of serum hormone [estradiol (E2), follicle stimulating hormone (FSH) and luteinizing hormone (LH) ] were compared between the two groups before and after treatment, and the adverse reaction of the two groups was recorded during treatment. Results The total effective rate of the experimental group (76.19%) was significantly higher than that of the control group (46.34%) (P < 0.05). After treatment, levels of E2 and FSH in the two groups were significantly higher than before treatment, and the experimental group was significantly higher than those in the control group (P < 0.05); the level of LH of the two groups decreased significantly, and the experimental group was lower than that of the control group (P < 0.05). The ovulation rate and pregnancy rate of the experimental group were significantly higher than those of the control group (P < 0.05). There was no significant difference in OHSS between the two groups (P > 0.05), and there was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Yangyin Shugan Capsules combined with Clomiphene Citrate is effective in the treatment of PCOS associated with infertility. It can improve the level of serum hormone, increase ovulation rate and pregnancy rate, and do not increase adverse reaction. It has a certain clinical value.

Dynamic change characteristics of thromboelastogram in pregnancy women′s

Objective To explore the dynamic change characteristics of thromboelastogram in pregnancy women′s. Methods From January to November 2018, a total of 150 pregnant women who were admitted to Liuzhou Traditional Chinese Medicine Hospital were in the pregnancy group, which were divided into the early pregnancy group (n = 47), the middle pregnancy group (n = 53) and the late pregnancy group (n = 50) according to the gestational week. The control group included 50 women who were not pregnant. TEG parameters and routine blood coagulation indexes were measured and compared among the control group, pregnancy group, and the postpartum period. Results The R and K values of the pregnancy group were significantly lower than those of the control group, while the Angle and MA values were significantly higher than those of the control group, with highly statistically significant differences (P < 0.01). The R and K values of the middle and late pregnancy group were significantly lower than those of the early pregnancy group, while the Angle and MA values were significantly higher than those of the early pregnancy group, with statistically significant differences (P < 0.05 or P < 0.01). The K value of the late pregnancy group was significantly lower than that of the middle pregnancy group, and the difference was highly statistically significant (P < 0.01). There were highly significant statistical differences in R, K, Angle and MA values at all postpartum time points (P < 0.01). With the development of postpartum time, R value and K value gradually increased, while Angle and MA value gradually decreased. There were highly statistically significant differences in ATPP, fibrinogen and Ti between pregnant groups (P < 0.01). Conclusion The indicators of TEG at different stages of pregnancy and postpartum have corresponding changes, which can reflect the changes of coagulation function dynamically.

Clinical effect of Phloroglucinol Injection combined with Gushen Antai Pills in treatment of threatened abortion

Objective To explore the effect of phloroglucinol injection combined with Gushen Antai Pills in the treatment of threatened abortion. Methods From February 2016 to April 2018, 109 pregnant women with threatened abortion admitted in Luohu District Women and Children′s Health Care Hospital of Shenzhen were selected, and they were divided into control group (54 cases) and study group (55 cases) according to the random number table method. The pregnant women in control group were treated with Phloroglucinol Injection, and the pregnant women in study group were treated with Phloroglucinol Injection combined with Gushen Antai Pills. The clinical efficacy was compared between the two groups, the serum index [human chorionic gonadotropin (β-HCG), estradiol (E2) and progesterone (P)] of the two groups were compared before and after treatment, the adverse reaction of the two groups during treatment was recorded. Results After treatment, the total effective rate of study group was higher than that of the control group (P < 0.05). After treatment, the levels of β-HCG, E2 and P were higher than before treatment (P < 0.05), and these in the study group were higher than those in the control group (P < 0.05). The disappearance time of clinical symptoms and uterine contraction in the study group were shorter than those in the control group (P < 0.05). There was no difference in the incidence of adverse reactions in the two groups during the treatment period (P > 0.05). Conclusion Phloroglucinol Injection combined with Gushen Antai Pills is effective in the treatment of threatened abortion. It can effectively improve the clinical symptoms of patients, improve the level of related serum indexes, and it does not increase the incidence of adverse reactions. It has a certain clinical value.

Observation and analysis of Metoclopramide combined with 5-HT3 receptor antagonist in patients with chemotherapy-induced nausea and vomiting

Objective To observe the effect of Metoclopramide combined with 5-HT3 receptor antagonists on chemotherapy-induced nausea and vomiting in patients with advanced cancer chemotherapy and observe their side effects. Methods Advanced cancer patients ineffective with Dolastron, Ondansetron, Tropisetron, and Palonosetron in Center of Oncology of Renming Hospital of Wuhan University from October 2016 to May 2018 were selected respectively, with 30 cases in each variety, named group A, B, C, and D group. Then they were treated combined with Metoclopramide. The occurrence of nausea and vomiting and other side effcts in each group were observed. Results There were no statistically significant differences in the effectiveness of the four groups in preventing acute and delayed nausea and vomiting (P > 0.05). The most common side effects of the four groups were headache, constipation, bloating, drowsiness, fatigue, dizziness, and thirst, but they could be alleviated after symptomatic treatment or withdrawal. Conclusion Among advanced cancer patients who were ineffective with 5-HT3 receptor antagonist alone, Metoclopramide combined with Dolastreline, Ondansetron, Tropisetron, and Palonosetron work in some patients and reduces the incidence of side reactions, which has positive clinical significance.

Efficacy of Apatinib Mesylate combined with hepatic artery embolization chemotherapy in the treatment of advanced primary liver cancer

Objective To observe the efficacy and safety of Apatinib Mesylate combined with hepatic arterial chemoembolization (TACE) in the treatment of advanced primary liver cancer. Methods From January 2015 to August 2016, 50 patients with advanced primary liver cancer treated in the Department of Oncology, Renmin Hospital of Wuhan University were selected as subjects. By retrospective analysis, patients were divided into combined group and control group, with 25 cases in each group. Patients in the combination group received TACE followed by sequential oral administration of Apatinib Mesylate, while patients in the control group received TACE alone. The clinical remission rate, survival time and incidence of adverse reactions were compared between the two groups. Results The clinical remission rate of the combined group was significantly higher than that of the control group (P < 0.05). The total survival time of the combined group was longer than that of the control group (P < 0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Apatinib Mesylate combined with TACE has the advantages of improving clinical effect, prolonging survival time and good safety in the treatment of patients with advanced primary liver cancer.

Expression and clinical significance of Axin and metastasis-associated gene 1 in colon cancer protein in gastric cancer tissues

Objective To investigate the expression of Axin and metastasis-associated gene 1 in colon cancer in gastric cancer tissues and their correlation with clinicopathological parameters. Methods From January 2015 to December 2017, 150 surgical specimens of patients with gastric cancer in the Qingdao Eighth People′s Hospital, Shandong Province were collected as the gastric cancer tissue and randomly pick one of them 70 cases as the paracancer tissues. The expression of Axin and MACC1 in the cancer and paracancer tissues were detected by immunohistochemistry, to analyze the relationship between the expression of Axin and MACC1 and the clinicopathological parameters of gastric cancer patients and their correlation. Results The positive expression rate of Axin in the gastric cancer tissue was lower than that in the paracancerous tissue, and the positive expression rate of MACC1 in the gastric cancer tissue was higher than that in the paracancerous tissue, the differences were highly statistically significant (P < 0.01). The expressions of Axin and MACC1 in gastric cancer tissues were related to the invasion depth, differentiation degree, lymph node metastasis and Duke stage of gastric cancer (P < 0.01), and the expression of Axin in gastric cancer tissues was negatively correlated with MACC1 (P < 0.05). Conclusion The decreased expression of Axin and the increased expression of MACC1 are both related to the development of gastric cancer. The early detection of these two indicators can provide an important basis for the prediction of the progress of gastric cancer. The expression of Axin is negatively correlated with MACC1, which may synergistically lead to low differentiation, high invasion and high metastasis of the tumor, suggesting that Axin and MACC1 may be potential targets for the treatment of gastric cancer.

Detection of serum microRNA in esophageal squamous cell carcinoma with neoadjuvant chemoradiotherapy sensitivity and its clinical application value

Objective To investigate the relationship between serum microRNA(miRNA) and neoadjuvant radiotherapy (NCRT) for esophageal squamous cell carcinoma and its predictive value. Methods From January 2015 to December 2016, 30 patients with preoperative NCRT cT3-4N0 esophageal squamous cell carcinoma admitted to Fuzhou General Hospital of Nanjing Military Command were selected. Peripheral blood of the patients were collected before NCRT. According to the postoperative pathological results, the patients were divided into the sensitive group and the resistance group, with 15 cases in each group. The differentially expressed serum mirnas in the NCRT sensitive group and the resistance group were detected by miRNA expression profile microarray. Then, the sample size was expanded for real-time fluorescence quantitative PCR to verify the initially screened serum mirnas. The relationship between the efficacy of miRNA and NCRT was analyzed and verified. Subject operating characteristic (ROC) curves were used to evaluate the value of miRNA in predicting the efficacy of NCRT. Results A total of 16 differentially expressed mirnas were detected in the serum of the two groups, among which miRNA-23, miRNA-211 and miRNA-323 were verified by qRT-PCR, and the differences were statistically significant (P < 0.05). The area under the curve of the short-term efficacy of the three factors and esophageal squamous cell carcinoma NCRT was 0.764, 0.880 and 0.707, respectively. Conclusion The evaluation of miRNA-23, miRNA-211 and miRNA-323 in the short-term efficacy of esophageal squamous cell carcinoma NCRT may have certain predictive value.

Effects of probiotic enteral nutrition on intestinal mucosal barrier function and inflammatory response in colon cancer patients

Objective To investigate the effects of probiotic enteral nutrition on intestinal mucosal barrier function and inflammatory response in colon cancer patients. Methods Ninety-four patients who underwent laparoscopic radical resection of colon cancer in the Second Hospital of Hebei Medical University from February 2016 to June 2018 were selected as subjects, and the patients were divided into control group and observation group with 47 cases in each group by random number table method. The group A was given early oral enteral nutrition after operation, and group B was given Clostridium butyricum viable tablets on the basis of group A. The levels of plasma endotoxin (ET), tumor necrosis factor-α (TNF-α), diamine oxidase (DAO) and C-reactive protein (CRP) were observed at 1st, 4th and 7th day after operation in the two groups. The operation time, intraoperative bleeding volume, anal exhaust time, 7-day internal defecation ratio and postoperative infection rate of the two groups were statistically analyzed. Results There was no significant difference in operation time and bleeding volume between the two groups (P > 0.05). The anal exhaust time of the observation group was lower than that of the control group, and the proportion of internal defecation in 7 days was higher than that of the control group (P < 0.05). The levels of ET, DAO, CRP and TNF-α in the observation group were lower than those in the control group at 4th and 7th day after operation (P < 0.05).The total incidence of postoperative infectious diseases in the observation group was lower than that in the control group (P < 0.05). Conclusion Early oral enteral nutrition combined with probiotics can protect the intestinal mucosal barrier, alleviate inflammatory reaction, promote the recovery of gastrointestinal function and reduce the incidence of postoperative infection in patients with colon cancer undergoing laparoscopic surgery, which is worthy of clinical application.

Chinical effects of minimally invasive percutaneous treatment of thoracolumbar spine fractures

Objective To observe the clinical effect of percutaneous minimally invasive pedicle screw fixation for the treatment of thoracolumbar fractures. Methods The clinical data of 108 patients with thoracolumbar spine fractures were selected who had received treatment in the Inner Mongolia People′s Hospital form July 2017 to July 2018. According to the different surgical methods applied, these patients were divided into control group and observation group with 54 cases in each group. The control group was underwent open surgery for pedicle screw fixation, and the observation group was underwent percutaneous minimally invasive surgery for pedicle screw fixation. The surgical indexes, spinal morphology and complications were compared between the two groups. Results There was no statistically significant difference between the two groups (P > 0.05). The incision size, blood loss, drainage volume, time for off-bed activities and for fracture line healing, and the length of hospital stay in the observation group were lower than the control group (all P < 0.05). In the observation group and the control group, the postoperative kyphosis Cobb angle and vertebral sagittal plane index and vercebral anterior margin height were improved compared with those before surgery (P < 0.05). The visual analague scale (VAS) score and spine function score in the observation group were lower than the control group (P < 0.05). The total incidence of complications in the observation group was lower than that in the control group (P < 0.05). There was no statistically significant difference in functional recovery grading between the two groups (P > 0.05). Conclusion Percutaneous minimally invasive pedicle screw fixation has the advantages of short operation time, less blood loss and rapid recovery, which can effectively restore the spinal structure of patients with thoracolumbar spinal fractures, reduce postoperative pain, improve spinal function and nerve function, and reduce the probability of postoperative complications, with definite curative effect.

Effects of different scoring systems on condition and prognosis of patients with emergency spontaneous nontraumatic cerebral hemorrhage

Objective To discuss rapid emergency medical grade (REMS), modified early warning score (MEWS) and acute physiology and chronic health evaluation system Ⅱ (APACHE Ⅱ) and the improvement of emergency (mEDICH) in emergency treatment of spontaneous cerebral hemorrhage grading scale non traumatic cerebral hemorrhage (sICH) patients and prognosis judgement of value. Methods A total of 201 cases of patients with sICH collected from September 2017 to August 2018, Department, Emergency of Renmin Hospital of Wuhan University, according to the hospital within 30 d death or death is divided into survival group and death group, and compared the four different points scoring system of case fatality rate difference was compared, and ROC curve was used to compare REMS, MEWS, APACHEⅡ and mEDICH score evaluation of the accuracy and clinical value of the prognosis of emergency patients with sICH. Results Within 30 d hospital death or death and survival of patients with sICH REMS, MEWS, APACHEⅡ and mEDICH score comparison, highly significant difference (P < 0.01). The higher the score of each scale was, the higher the fatality rate was, and the difference between each score segment was highly statistically significant (P < 0.01). REMS, MEWS, APACHE Ⅱ and mEDICH score area under the ROC curve were 0.733, 0.777, 0.833 and 0.899, respectively. Area under the ROC curve of mEDICH score significantly greater than the REMS, MEWS and APACHE Ⅱ (P < 0.01). Conclusion REMS, MEWS, APACHE Ⅱ and mEDICH scores are good for emergency treatment of sICH patients was dead or not within 30 d to predict the risk of death, the predictive ability of mEDICH is the best.

Analysis of postoperative infection and related factors in patients with cardiothoracic surgery ICU

Objective To investigate postoperative infection and related risk factors in patients with cardiothoracic surgery ICU. Methods A retrospective analysis was made on 240 patients admitted to ICU of Cardiothoracic Surgery, First Affiliated Hospital of Hunan University of Traditional Chinese Medicine from July 2016 to July 2018. The clinical data (including age, cardiopulmonary bypass, time of tracheal intubation, time of drainage tube placement, time of catheter placement, operation time, blood transfusion, time of deep vein catheterization) and monitoring funds of nosocomial infection were collected. The data were systematically investigated and analyzed. Results Hospital infection occurred in 38 of 240 patients, with an infection rate of 15.83%. The most common site of infection is the respiratory tract, a total of 25 cases. Fifty-nine strains of pathogens were detected, including 34 Gram-negative bacteria, 15 Gram-positive bacteria, and 10 fungi. Multivariate Logistic regression analysis showed that age, extracorporeal circulation, tracheal intubation time, and drainage tube placement time were independent risk factors for postoperative infection, and the differences were statistically significant (P < 0.05). The main Gram-negative bacteria were Acinetobacter baumannii and Klebsiella pneumoniae, which were more resistant to Cefixime, Imipenem, Ciprofloxacin, Gentamicin, Ampicillin and Aztreonam. Gram-positive bacteria were mainly Staphylococcus haemolyticus, Staphylococcus epidermidis and Staphylococcus hemorrhagicus. Gram-positive bacteria mainly include Staphylococcus haemolyticus, Staphylococcus epidermidis, and Staphylococcus aureus, which were sensitive to Vancomycin and Linezolid, but they were resistant to Amoxicillin, Azithromycin, Deoxycephalosporin, Meropenem, and Cefixime, with higher resistance rate. Conclusion The infection rate of ICU patients in cardiothoracic surgery is high, and there are many factors leading to infection. The clinical intervention should be given. The drug resistance rate of pathogenic bacteria is relatively high. Clinical attention should be paid to the detection of pathogenic microorganisms, and rational use of antibiotics should be based on the results of bacterial culture and drug sensitivity.

Relationship between serum FGF23 and FA level and coronary artery calcification in patients with maintenance hemodialysis

Objective To investigate the relationship between serum fibroblast growth factor 23 (FGF23) and fetoprotein A (FA) levels and coronary artery calcification in patients with maintenance hemodialysis. Methods From April 2016 to April 2017, 97 cases of maintenance hemodialysis patients admitted to the Second Affiliated Hospital of Xi′an Jiaotong University were selected. All patients underwent MSCT, and according to coronary artery Agaston score (CACs), the patients were divided into non-calcification group (CACs<10 points) 21 cases, mild calcification group (11 points≤CACs<100 points) 33 cases, moderate calcification group (100 points≤CACs<400 points) 24 cases, and severe calcification group (CACs>400 points) 19 cases. Basic data and biochemical indexes of the patients in each group were compared, and correlation analysis was conducted. Results The age of patients in the severe calcification group was significantly higher than that in the non-calcification group, the mild calcification group and the moderate calcification group, and the duration of dialysis was longer than that in the non-calcification group, the mild calcification group and the moderate calcification group, and the duration of dialysis in the moderate calcification group was longer than that in the non-calcification group, with statistically significant differences (all P < 0.05). There was no significant difference in the prevalence of diabetes, hypertension and gender composition among the groups (P > 0.05). The levels of serum phosphorus and FGF23 in the non-calcification group, mild calcification group, moderate calcification group and severe calcification group presented a gradually increasing trend, while the level of serum FA presented a gradually decreasing trend, the differences between the groups were statistically significant (all P < 0.05). Blood urea nitrogen, serum creatinine, glomerular filtration rate, fasting blood glucose, total cholesterol, triglyceride, and serum calcium were compared between groups, and the differences were not statistically significant (P > 0.05). Pearson correlation analysis showed that the degree of coronary artery calcification in patients with maintenance hemodialysis was positively correlated with age, duration of hemodialysis, serum phosphorus and serum FGF23 level, while negatively correlated with serum FA level (all P < 0.05). Conclusion Serum FGF23 and FA levels are closely related to coronary artery calcification in patients with maintenance hemodialysis, and with the increase of serum FGF23 level and the decrease of FA level, the degree of coronary artery calcification in patients with maintenance hemodialysis becomes more serious.

Clinical effect of core muscle training combined with acupuncture in treatment of ischemic stroke patients with hemiplegia

Objective To study the clinical effect of core muscle training combined with acupuncture in treatment of ischemic stroke patients with hemiplegia. Methods A total of 100 cases of ischemic stroke patients with hemiplegia were selected in Danzhou People′s Hospital of Hainan Province from June 2016 to April 2017, and they were randomly divided into control group and observation group with 50 cases in each group according to the random number table method. The control group was given core muscle training, and the observation group was treated with acupuncture on the basis of control group, the walking parameters, lower extremity motor function,resistance index (RI), pulsatility index (PI) and mean blood flow velocity (Vm) of bilateral middle cerebral arteries was compared between the two groups before treatment and 1 month after treatment. The quality of life was evaluated by Barthel index (BI) scale. Results The simplified Fugl-Meyer Motor Function Scale (FMA) and Berg Balance Scale (BBS) scores of the two groups were higher than those of the control group after 1 month of treatment, and the observation group was higher than those of the control group (P < 0.05); the timed standing walk test (TGUT) was lower than that of the control group (P < 0.05). The walking speed, stride length, healthy side stride length, affected side stride length and leg support period of the two groups improved 1 month after treatment, and those in the observation group were higher than the control group (P < 0.05). One month after treatment, Vm in the observation group was higher than that in the control group (P < 0.05), while RI and PI in the observation group was lower than those in the control group (P < 0.05). The BI of the two groups increased 1 months after treatment, and the observation group was higher than that of the control group (P < 0.05). Conclusion Core muscle group training combined with acupuncture can effectively improve cerebral blood flow, walking parameters, lower extremity motor function and quality of life in Ischemic stroke patients with hemiplegia, the clinical value is excellent.

Application effect of autologous blood continuous transfusion machine in dangerous placenta previa

Objective To analyze the application effect of autologous blood continuous transfusion machine in caesarean section for patients with dangerous placenta previa. Methods A retrospective analysis was made on 120 cases of dangerous placenta previa diagnosed in the First Affiliated Hospital of Xinjiang Medical University from July 2016 to August 2018. They were divided into group A (n = 60) and group B (n = 60) according to whether autologous blood continuous transfusion machine was used during operation. Sixty patients in group A used autologous blood continuous transfusion machine during operation, while 60 patients in group B did not use autologous blood continuous transfusion machine. The intraoperative bleeding volume, blood transfusion and the changes of blood test indexes were compared between the two groups. Results There was no difference in intraoperative bleeding volume between the two groups (P > 0.05), The volume and rate of intraoperative transfusion of allogeneic blood products in group A were lower than those in group B (P < 0.05), and the preoperative preparation time in group A was lower than that in group B (P < 0.05). The levels of blood platelets (PLT), hemoglobin (Hb), and hematocrit (HCT) in the two groups were significantly lower than those before operation (P < 0.05), but there was no difference between the two groups (P > 0.05). Conclusion Continuous autologous blood transfusion machine can effectively reduce the amount and ratio of blood products transfusion, maintain circulating blood volume and save blood resources in the caesarean section in patients with dangerous placenta previa.

Relationship between serum chemerin level and epicardial adipose tissue volume and coronary artery calcification in maintenance hemodialysis patients

Objective To study the relationship between serum chemerin and epicardial adipose tissue volume, coronary artery calcification in maintenance hemodialysis patients. Methods From February 2015 to February 2018, 96 patients with maintenance hemodialysis admitted to NO.2 People′s Hospital of Fuyang City were selected as the study group. Another 50 healthy persons who received physical examination in the same period were recorded as control group. The levels of biochemical indexes in the two groups were compared. At the same time, the study group was divided into low-risk group (50 cases), medium-risk group (27 cases) and high-risk group (19 cases) according to Framingham risk score. The volume of epicardial adipose tissue, coronary artery calcification score and serum chemerin level in the three groups were compared, and the correlation analysis was made. Results The levels of chemerin, neutrophil percentage, neutrophils/lymphocytes, C-reactive protein, urea, creatinine, fasting blood glucose in the study group were higher than those in the control group, the platelet count was lower than that in the control group, the differences were highly statistically significant (P < 0.01). The volume of epicardial adipose tissue, coronary artery calcification score and serum chemerin level in low-risk group, medium-risk group and high-risk group showed a gradual increase trend, the differences were highly statistically significant (P < 0.01). The serum chemerin level of maintenance hemodialysis patients was positively correlated with the volume of epicardial adipose tissue and coronary artery calcification score (r = 0.713, 0.689, P < 0.05). Conclusion The elevation of serum chemerin in maintenance hemodialysis patients is closely related to epicardial adipose tissue and coronary artery calcification. Detection of serum chemerin can predict the risk of cardiovascular disease in patients.

Relationship between neonatal blood glucose and glycated albumin, insulin and C-peptide in neonates born by gestational diabetes women

Objective To investigate the relationship among blood glucose and glycated albumin, insulin and C-peptide in neonates born by gestational diabetes (GMD) women. Methods From February 2017 to April 2018, in Baoding First Central Hospital, 293 neonates born were selected, according to the neonatal blood glucose and whether the neonatal mother had GDM or not, the neonates were divided into GDM mothers′ neonates hypoglycemia group (74 cases), GDM mothers' normal blood glucose group (68 cases), non-GDM mothers′ neonatal hypoglycemia group (16 cases), non-GDM mothers' normal blood glucose group (135 cases). Glycated albumin, C-peptide, fasting insulin level (FINS), insulin sensitivity index (ISI) and insulin resistance index (HOMA-IR) in groups were detected and compared, and the correlation between the above indicators and neonatal blood glucose was analyzed. Results The incidence of neonatal hypoglycemia in neonates born by GDM mothers was higher than that in neonates born by non-GDM mothers (P < 0.05). The levels of FINS, ISI, HOMA-IR and C-peptide in GDM mothers′ neonates hypoglycemia group were higher than those in GDM mothers' normal blood glucose group, the differences were statistically significant (P < 0.05). There was no significant difference in glycated albumin, FINS, ISI, HOMA-IR and C-peptide in non-GDM mothers′ neonatal hypoglycemia group and non-GDM mothers′ normal blood glucose group (P > 0.05). Further analysis showed that the above indicator were negatively correlated with FINS, ISI, HOMA-IR and C-peptide in neonates born by GDM mothers (r = -0.581, -0.712, -0.642, -0.621, P < 0.05). There was no significant correlation between glycosylated serum albumin and blood glucose (P > 0.05). Conclusion Neonates born by GDM mothers are prone to hypoglycemia, and the correlation between neonatal blood glucose and insulin and C-peptide level is significantly negative, with neonatal glycated albumin alone can not estimate whether hypoglycemia occurs in neonates after birth.

Analysis of the availability of oral antidiabetic drugs in public medical institutions in Shandong Province

Objective To analyze and evaluate the availability and equipping rate of oral antidiabetic drugs in public medical institutions at all levels in Shandong Province. Methods According to the standardized survey method of World Health Organization / Health Action International (WHO/HAI), the equipping conditions and availability of 7 commonly used oral antidiabetic drugs in 39 public medical institutions of the first, second, third levels in 3 cities of Shandong Province were analyzed and investigated. Results The equipping rate and availability of oral antidiabetic drugs in tertiary hospitals and second-class hospitals were all higher than those in primary health care institutions. The availability of Repaglinide (36.8%) was the highest in original drugs, and the availability of Glipizide (13.4%) was the lowest, the original drugs of Glibenclamide couldn′t be obtained. The availability of Metformin was the highest (86.8%) in generic drugs, and the availability of Glibenclamide was the lowest (5.3%). In tertiary hospitals, original drugs (77.8%) had higher equipping rate than generic drugs (57.1%). In primary medical institutions, original drugs (8.6%) had lower equipping rate than generic drugs (43.3%), and the equipping rates of both of them were lower. Conclusion The availability and equipping rate of oral antidiabetic drugs in general hospitals in Shandong Province are all higher, which in primary health care institutions are all lower. In order to meet the medical needs of diabetic patients, it is necessary to take various measures to improve the availability of oral antidiabetic drugs in primary health care institutions and the quality of generic drugs.

Effect of Ginkgo dipyridamole combined with Xingnaojing in patients with traumatic intracranial hemorrhage and its effect on neurological function recovery

Objective To explore the efficacy of Ginkgo dipyridamole combined with Xingnaojing in patients with traumatic intracranial hemorrhage and its effect on neurological function recovery. Methods Eighty patients with traumatic cerebral hemorrhage admitted to the Cangzhou Hospital of integrated TCM-WM·Hebei from January 2017 to May 2018 were divided into control group (n = 40) and study group (n = 40) according to the random number table method. The patients in the control group were treated with Xingnaojing, and the patients in the study group were treated with Ginkgo dipyridamole on the basis of the control group. Both groups were treated for 14 days. The clinical effects of the two groups were compared. The levels of nerve growth factor (NGF), human brain-derived neurotrophic factor (BDNF), neuropeptide Y (NPY), National Institute of Health Stroke Scale (NIHSS) and Glasgow Coma Scale (GCS) scores were compared before and after treatment. At the same time, the adverse reactions of the two groups were observed during the treatment. Results The total effective rate of the study group was significantly higher than that of the control group (P < 0.05). The levels of NGF, BDNF and GCS score in the two groups were higher than those in the control group before treatment, and the study group was higher than the control group (P < 0.05 or P < 0.01). The NPY level and NIHSS scores of the two groups were lower than those before treatment, and the study group was lower than the control group (P < 0.05 or P < 0.01). There were no serious adverse reactions during the treatment period in both groups. Conclusion Ginkgo dipyridamole combined with Xingnaojing has definite curative effect on traumatic cerebral hemorrhage. It can significantly improve the neurological function of patients with good safety and has certain clinical application value.

Effects of Cetirizine drops combined with Budesonide Suspension on FENO and SOD in children with acute asthma

Objective To investigate the effect of Cetirizine drops combined with Budesonide Suspension aerosol inhalation on acute attack of asthma in children and its effects on serum superoxide dismutase (SOD) and exhaled nitric oxide (FENO). Methods A total of 140 children with acute asthma exacerbations from July 2014 to February 2017 in Tangshan People′s Hospital of Hebei Province were enrolled and randomly divided into control group (n = 71) and observation group (n = 69). The control group received Budesonide Suspension inhalation therapy on the basis of symptomatic treatment, while the observation group received Cetirizine drops combined with Budesonide Suspension aerosol inhalation treatment on the basis of symptomatic treatment. The total effective rate, the first second forced expiratory volume (FEV1), the percentage of FEV1 in the predicted value (FEV1%), serum SOD activity and FENO content were compared between the two groups. Results The total effective rate of the control group was lower than that of the observation group (P < 0.05). There was no significant difference in FEV1, FEV1%, serum SOD activity and FENO content between the two groups before treatment (P > 0.05). After treatment, FEV1, FEV1% and serum SOD activity in the two groups were higher than those before treatment, while FENO content was lower than that before treatment, the differences were statistically significant (P < 0.05). After treatment, the FEV1, FEV1% and SOD activity in the control group were lower than those in the observation group, while the content of FENO in the control group was higher than that in the observation group, with statistical significance (P < 0.01). Conclusion Compared with Budesonide Suspension alone, Cetirizine drops combined with Budesonide Suspension can effectively alleviate acute attack of asthma in children, improve lung function, increase serum SOD activity and reduce FENO content.

Effect of interferon α-1b sequential Lamivudine combined therapy on T lymphocyte subsets, complement levels and liver function in children with chronic hepatitis B

Objective To investigate the effects of interferon α-1b sequential Lamivudine on the expression of T lymphocyte subsets and complement in children with chronic hepatitis B, and to evaluate liver function. Methods A total of 136 children with chronic hepatitis B admitted to the Shijiazhuang Fifth Hospital (“our hospital” for short) from November 2015 to December 2017 were selected as subjects. They were divided into study groups according to the random number table method, and 68 cases in each group. The control group received only intramuscular injection of interferon α-1b. The study group was treated with Lamivudine combined interferon α-1b. The T lymphocyte subsets (CD4+, CD8+, CD4+/CD8+), CD8+ T cell PD-1, Tim-3 expression, complement (C3, C4) levels were observed before and after treatment in two groups. The levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST) and total bilirubin (TBil) were observed before and after treatment in both groups. Results After treatment, the levels of CD4+, CD4+/CD8+ in the study group were higher than those in the control group, and the levels of CD8+ in the study group were lower than those in the control group (P < 0.05). After treatment, the levels of CD8+ T cell PD-1, CD8+ Tim-3 in the study group were lower than those in the control group, while the levels of C3 and C4 in the study group were higher than those in the control group (P < 0.05). After treatment, ALT, AST and TBil in the two groups were lower than those before treatment, and the differences between the study group and the control group were statistically significant (P < 0.05). Conclusion Lamivudine combined with interferon α-1b in the treatment of children with chronic hepatitis B can significantly improve the T lymphocyte subsets of children, HBV DNA, HBsAg, HBeAg levels are significantly improved, with significant anti-fibrosis effect.

Meta analysis on effect of web-based continuous nursing care on negative emotions and quality of life with breast cancer patients after operation

Objective To evaluate the effect of web-based continuous nursing care on negative emotions and quality of life with breast cancer patients after operation. Methods Randomized controlled trials (RCTs) about the impact of continuous nursing on negative emotions and quality of life with breast cancer patients after operation were searched in Chinese and English databases by computer. The searching time was from foundation to August 2018. Two researchers surveyed the literature, evaluated the quality and extracted the data. RevMan5.3 software was used to analyze the RCT which met the quality standards. Results A total of 13 RCTs including 1272 cases with breast cancer. The quality level of the included literature was above B level, and containing 2 high-quality studies. The results of Meta analysis showed that after the implementation of web-based continuous nursing, the scores of anxiety [SMD = 1.07, 95%CI (0.12, 2.02), P = 0.03] and depression [SMD = 1.14, 95%CI (0.28, 2.00), P = 0.01] in the intervention group were lower than those in the control group, the quality of life [SMD = 0.87, 95%CI (0.58, 1.15), P < 0.000 01] were significantly improved. Conclusion The web-based continuous nursing can effectively alleviate the anxiety and depression of breast cancer patients after operation and improve their quality of life, and is worthy of studying and applying further.

Prevention countermeasures research on control of infection in nursing home in a general hospital

Objective To explore the prevention and control countermeasures on nosocomial infection in general hospital, to reduce the incidence of nosocomial infection. Methods A total of 228 patients from October, 2016 to June, 2018 who were hospitalized in a general hospital nursing home were selected for statistical analysis of the patient′s age, basic diseases, length of stay, antibiotic medication, etc, and the risk points of infection prevention and control were found to explore the prevention and control methods and countermeasures of hospital infection. Results The points of infection prevention and control risk for inpatients were as following: most of them were elderly patients; long hospital stay and high frequency; severe and complex condition; there were many basic diseases in inpatients; antimicrobial drug use and invasive procedures increase the risk of infection; the proportion of unprofessional nurses was higher. Conclusion In view of the risks above, the following countermeasures are proposed: strengthening multidisciplinary collaboration; standardize the operation of diagnosis and treatment, reduce the invasive operation; strengthen environmental cleaning and disinfection isolation; standardize the nursing management; give full play to the role of part-time quality control; training, supervision and exercise should be strengthened to gradually prevent and control the risk factors of hospital infection and finally reduce the incidence of hospital infection.

A brief discussion on terminal neuritis from emotion and syndrome differentiation

The clinical symptoms of peripheral neuritis mainly are symmetrical dyskinesia and abnormity of sense at the extremities, and autonomic nervous dysfunction. In traditional Chinese medicine, it is classified as "blood-arthralgia". By combining the theory of scholars of the past with our own experience, we think that the pathogenesises of the disease are mainly "deficiency" "pathogenic factors" and "blood-stasis". In addition, the occurrence of the disease caused by emotional factors is due to the disorder of Qi activities and visceral functions, which leads to the deficiency of healthy Qi, invasion of pathogenic factor and endogenesis of blood stasis. This article analyzes the etiology and pathogenesis of this disease comprehensively and reports a medical case of this disease treated from emotional factors, which providing a new idea for treating peripheral neuritis in TCM.

Analysis on the construction of hospital comprehensive risk management system

The continuous development of the social economy has made the people′s health awareness constantly increase, and the demand for medical services has also increased. With the continuous advancement of medical system reform, public hospitals face more and more risks and the risks become more and more complex. The establishment of the hospital internal control system has a certain foundation. Discussion on incentive mechanism of scientific and technological talents in medical research institutions under the background of classification reform of institutions

As an important part of public welfare scientific research institutions, medical research institutions are an important force for national medical science and technology innovation and the base of scientific and technological talents. Initiative and creativity of talents are related to the promotion of scientific and technological innovation capabilities, the development of public welfare, the implementation of healthy China strategy and national innovation-driven development strategy, and the overall development of economy and society. Under the background of the classification reform of public institutions, there are still some major problems in the incentives for scientific and technological talents in medical research institutions in China, such as relatively low salary satisfaction, unclear career development prospects, and insufficient autonomy of scientific and technological talents. The reasons can be summarized as institutional constraints on salary growth in scientific research institutions, imperfect construction of scientific and technological personnel and management of scientific research projects. To this end, the construction of the incentive mechanism for medical science and technology talents is explored from the aspects of improving the salary system, expanding the career development, optimizing the environment for the growth of scientific and technological talents, and improving and innovating the management stipulations.

Development of project-driven basic medical resource datebase

This article discusses the methodology of developing the project-driven basic medical resource database. This on-line project is established based on the integration of medical foundation subjects and project-driven teaching and learning. With features of organ system focused, combination of classroom and informationized teaching and learning methodologies, integration of experiment and practice, and cooperation and sharing each other for the project, the database consists of seven modules including learning guide for students, courcewares, picture recognitions, animations, micro classes, case studies and tests & examinations, which will be helpful for students to learn basic medical subject related macrostructure and microstructure, organ morphology and function, embryo and birth, and extend to their specialized courses.
