4 The application of conscious sedation combined with local anesthesia bronchoscopy on lung cancer patients during the perioperative period

Objective To investigate the effect of conscious sedation with local anesthesia bronchoscopy on lung cancer patients during the perioperative period and to find a better method for bronchoscopy. Methods Form March 2015 to December 2017, 326 patients received bronchoscopy during the perioperative period of surgery. These patients were divided into two groups according to the type of anesthesia: the local anesthesia group (Group A, 154 cases), and the conscious sedation with local anesthesia group(Group B, 172 cases). Preoperative evaluation, intraoperative monitoring, postoperative survey, and follow-up were conducted to assess the patients′ satisfaction, the safety of the operation, and the effect on postoperative rehabilitation of the surgery. Results No statistically significant difference of two groups was found in blood oxygen saturation, dyspnea, chest pain etc (P > 0.05), while there was statistically significant differences in the examining time, heart rate changes, the number of attempt times before the bronchoscope to pass through the glottic, patients′ willingness for re-examination, and the bad memory etc of two groups (P < 0.01). Conclusion Conscious sedation combined with local anesthesia method can reduce the patients′ fear for bronchoscopy in lung cancer surgery, improve the patients′ satisfaction and tolerance, and significantly reduce the incidence of complications. Therefore it has great application value.

8 Effect of TanshinoneⅡA Sulfonate Injection against HK-2 apoptosis induced by Vancomycin and the probable mechanism

Objective To explore the effect of TanshinoneⅡA Sulfonate Injection against HK-2 cell damage induced by Vancomycin and the probable mechanism. Methods HK-2 cells were cultured in vitro till the number was up to 1×106 /mL, randomly divided into five groups: control group, VAN group, low dose group of TSⅡA Sulfonate Injection, middle dose group of TSⅡA Sulfonate Injection, hight dose group of TSⅡA Sulfonate Injection. Except for the control group adding PBS, other groups were added 100 μg/L VAN for 24 hours. Then, the control group and VAN group were respectively added to the same volume of PBS, and the high dose group, middle dose group and low dose group of TSⅡA respectively added to different concentrations of TSⅡA. The cell viability was detected with MTT method, the activity of LDH were measured and the cell viability was detected with the content of LDH. DCFH-DA was used as the fluorescence probe to detect the level of ROS by a fluorescence microplate reader. Annexin V-FITC/PI double dyeing method was used to detect apoptosis rate. Results Compared with the control group, the survival rate of the cells and the activity of CAT were decreased, meanwhile the level of ROS, the activity of LDH and apoptosis rate were increased in the VAN group (P < 0.05). Compared with the VAN group, the survival rate of the cells and the activity of CAT were increased, meanwhile the level of ROS, the activity of LDH and apoptosis rate were decreased in the low, middle and hight dose group of TSⅡA Sulfonate Injection (P < 0.05), and there was a concentration dependence. Conclusion TSⅡA Sulfonate Injection can promote the proliferation of HK-2 cells in vitro, and improve the biochemical parameters and enzyme levels. The results suggest that TSⅡA Sulfonate Injection has a protective effect on HK-2 cell, and the protective mechanisms may be related with its antioxidant effect.

12 Study on the correlation between the carotid I/M ratio and renal atherosclerosis in experimental rabbit carotid atherosclerosis models

Objective To analyze the correlation between the change in carotid intima / media membrane (I/M) ratio and renal atherosclerosis in experimental rabbit carotid atherosclerosis models. Methods Forty-eight rabbits were randomly divided into the blank group, the control group and the model group by the random number table method, with 16 rabbits in each group. The blank group was not given any intervention, which was given basal feed. The control group was given high-fat diet of 120 g/d. The model group was given 7 d of high-fat diet and then the rabbits were changed into bilateral carotid atherosclerosis models through carotid intima air drying after 12 h of fasting (without fasting water). The bilateral operative carotid arteries and renal artery specimens of all rabbits were obtained to analyze the I/M ratio after routine paraffin embedding, sectioning and HE staining. Serum levels of triglyceride (TG), total cholesterol (TC) and C-reactive protein (CRP) were determined at the end of the experiment. Urinary microalbumin / urine creatinine (UACR) was analyzed and correlation analysis was performed. Results HE staining under light microscope was normal in the carotid artery and renal artery of rabbits in the blank group. In the control group, there was slight intimal hyperplasia in the carotid artery and slight hyperplasia and stenosis in the renal artery, without obvious lipid deposition. In the model group, there was obviously proliferated and thickened carotid intima, a large number of foam cells accumulated in the intima, extensively proliferated intima of the renal artery and seriously narrowed vessels, and thickened intima was accumulated foam cells at high magnification. Compared with the blank group, the carotid artery, renal artery I/M ratio in the control group and model group were significantly increased, serum TG, TC, CRP and UACR were significantly increased, and the carotid artery, renal artery I/M ratio and serum TG, TC, CRP and UACR in the model group were significantly higher than those of control group, the differences were statistically significant (P < 0.05). The I/M ratio of carotid artery in rabbit carotid atherosclerosis models was positively correlated with the I/M ratio of renal artery, CRP and UACR (P < 0.05), but there was no significant correlation with TG or TC (P > 0.05). Conclusion Carotid atherosclerosis can promote the occurrence of renal atherosclerosis, which may be related to inflammatory reactions, vascular endothelial cell injury and renal vascular diseases.

17 Preparation of the macroporous polysaccharide hemostatic complex materials and study on its performance in vitro

Objective To evaluate the hemostatic effect of the macroporous polysaccharide hemostasis complex materials preliminarily, and to investigate the performance of the materials in vitro. Methods The hemostatic materials were prepared by γ-irradiation crosslinked method, and its structure, degradation in vitro and clotting performance were characterized by infrared spectrum and physicochemical test. Results The results showed that the crosslinked carboxymethylated-chitosan molecules had been made after the materials were irradiated, and its water absorption was of 3456%. The biodegradable degradation of the materials in lysozyme solution was 68% after 3 days and the degradation completed within 20 days. The clotting time was (25.33±2.45) s, which indicated the material adhered more red blood cells and platelets and showed the stronger blood clots. Conclusion The macroporous polysaccharide hemostasis complex materials prepared by γ-irradiation crosslinked method possess great solution absorption performance and blood coagulation performance, which has good application prospect in wound hemostasis.

21 Research progress on relationship between heart failure and sympathetic nerves

When heart failure occurs, the body starts to compensate for the humoral mechanism of the nerves, including sympathetic enhancement and activation of the renin-angiotensin-aldosterone system. Therefore, the sympathetic nervous system and the occurrence and development of heart failure are closely linked. For the treatment of heart failure, treatment of drug and surgical treatment of sympathetic nerve have a certain effect. This article focuses on the relationship between sympathetic nerves and heart failure, development and treatment, to improve the treatment of heart failure on the relationship between sympathetic nerves and heart failure.

25 Research progress on the prognosis of high-risk human papillomavirus infection after conization of cervical lesions and its related factors

High-risk human papillomavirus (HR-HPV) infection is a necessary condition in the occurrence and development of cervical squamous intraepithelial lesion (SIL) and cervical cancer. There are cases that high-grade squamous intraepithelial lesion (HSIL) and cervical squamous cancer at stageⅠA1 remain or reoccur, or even become more severe after conization. The prognosis of HR-HPV has high correlation with the prognosis of cervical lesion. The treatment and follow-up of HSIL and cervical squamous cancer at stageⅠA1 after conization should be consulted in the monitoring conditions of HR-HPV. It is worthwhile making further study in the specific practical significance of HR-HPV infection indices for the prognosis of cervical lesion during follow-up, as well as the high-risk factors that influence the prognosis of HR-HPV.

29 Research progress of Wnt signaling pathway and osteoporosis treatment

Adult bone is a multifunctional organ that has been continuously reshaped. The homeostasis of bone requires the balance between osteoclast bone absorption and osteogenesis. If this imbalance can lead to osteoporosis, sclerosing osteopathy and other bone diseases. In order to find effective and safe treatment methods to regulate bone formation, the molecular mechanism of bone cell differentiation and activity must be elucidated. Gene researches in humans and mice have established Wnt signaling as a key mechanism for stimulating osteoblast differentiation and activity.This paper reviews the related mechanisms of Wnt signaling pathway and bone formation and the treatment of osteoporosis.

33 Research progress of treatment with Chinese medicine for insomnia rats induced by PCPA

As a common disorder, insomnia often brings serious effects on people′s normal work and life, and even induces a variety of physical diseases. Although sedative-hypnotic drugs like benzodiazepine and barbiturate effect quickly, they can cause unavoidable side effects. Treatment of insomnia with Chinese medicine has characteristics of no addiction and dependence. PCPA is a universal drug used in insomnia model. This article collects and arranges the experimental literature documents about treatment with Chinese medicine for insomnia rats induced by PCPA, and discuss from animal behavior, and mechanism of action in order to provide reference for researchers.

41 Current status and progress of nuclear medicine and molecular imaging in the diagnosis of pulmonary embolism

In recent years, with the renewal and comprehensive application of imaging diagnostic techniques, the diagnostic rate of pulmonary embolism has been greatly improved, but common pulmonary embolism (PE) still have some limitations. Since the prospective study of pulmonary embolism diagnosis (PIOPED) since its establishment, imaging equipment and radiopharmaceuticals have developed rapidly and further improved pulmonary ventilation perfusion (V/Q) imaging techniques. Single photon emission computed tomography (SPECT) combination of SPECT and low dose CT (SPECT/CT) has been widely used in daily practice. Positron emission computed tomography has recently been proposed (PET) lung perfusion/ventilation imaging, however, has not yet been clearly confirmed in large-scale prospective studies. In this paper, we will discuss various imaging methods such as V/Q SPECT, SPECT/CT and V/Q PET/CT, and summarize the present situation and development trend of molecular imaging in the study of in vivo characteristics of venous thromboembolism.

45 Survey analysis of antiepileptic drugs prescription and off-label use in children with epilepsy

Objective To summarize the situation of antiepileptic drugs (AEDs) administration and off-label use in epileptic children hospitalized in Children′s Hospital, Capital Institute of Pediatrics (CIP Children Hospital), and investigate the reason for off-label use, and provide evidence for the appropriate administration of AEDs in epileptic children. Methods Demographic and clinical diagnosis and therapy information of epileptic children, including gender, age, seizure types, AEDs administration and and so on, were collected in CIP Children Hospital from January 2011 to December 2015. Based on the label and instruction of AEDs, whether the AEDs administration was off-label use of which types were categorized into off-age, off-seizure types, off-dosage, off-mono/add-on therapy and off-administration frequency were evaluated, and the situation of off-label use of AED was statistically analyzed. Results Totally 865 epileptic children were collected, including 500 males and 365 females. The median age was 2.3 years old. Among them, 724 epileptic children received AEDs therapy, while the other 141 did not (including 82 children with Adrenocorticotropic Hormone, Prednisone, and/or Methylprednisolone therapy). 1120 person-times of AEDs were administrated. The numbers of epileptic children who received 1, 2, 3 and ≥4 AEDs were 492, 125, 55 and 52 cases. The percentage rank of types of off-label use was off-age (35.8%), off-mono/add-on therapy (19.7%), off-seizure types (14.6%), off-dosage (4.5%) and off-administration frequency (2.3%). Moreover, top 3 percentage of AEDs in off-age were Topiramate (72.9%), Levetiracetam Tablets (67.0%), and Oxcarbazepine Liquid (56.7%); top 3 percentage of AEDs in off-seizure types were Levetiracetam Liquid (51.9%), Levetiracetam Tablets (40.8%), Topiramate (26.3%); top 3 percentage of AEDs in off-mono/add-on therapy were Levetiracetam Liquid (81.4%), Levetiracetam Tablets (55.5%), Topiramate (23.7%). Conclusion The off-label use of AEDs is widespread in epileptic children, especially in off-age. The most percentage of off-label use in AEDs were Levetiracetam and Topiramate. The main reason for the off-label use in AEDs is the complication of children epileptic therapy. When the administration of AEDs is an off-label use, the adverse effects and benefits should be evaluated, so that get optimal balance of drug efficacy and safety and promote clinical appropriate drug administration.

49 Comparative analysis on development status quo between private and public nursing institution for the aged in Guangzhou

Objective To understand the current situation of nursing institution for the aged in Guangzhou, and compare the different and problems between private and public nursing institution so as to make relevant suggestions. Methods Descriptive analysis method was used to analyze data among number of institutions, beds area, investment and nursing assistant collected from the Civil Affairs Bureau of Guangzhou in 2015. An interview survey was conducted among infrastructure conditions, nursing assistants and problem from June to October 2016. Results The total number of the nursing institution for the aged were 177, and the beds were 48 511. Private nursing institution accounted for 63.3% of the total number of the nursing institution in Guangzhou, and the beds accounted for 73.0%. The area per capita of the public nursing institution for the aged was 73.19 m2 and the private nursing institution was 24.78 m2. In addition, the floor area per capita of the public nursing institution for the aged was 30.87 m2 and the private nursing institution was 22.76 m2. The construction investment of the private nursing institution for the aged was higher than the public pension agency of 1.8 893 942 billion Yuan, society or public pension agency high of 318.7384 million Yuan, but the lottery welfare fund was lower of 55.908 45 million Yuan. At last, the proportion was 1∶4.25 between nursing assistant and the elderly in public nursing institution for the aged and it was 1∶5.70 in the private nursing institution for the aged. Conclusion The private nursing institution has become the mainstay of pension service, and the overall investment of private nursing institution is far exceeds the public nursing institution, but the development of private nursing institutions are greatly influenced by land. In addition, the proportion between nursing assistant and the elderly in private  nursing institution is lower than in public nursing institution. To solve above problems, governments should let go of pension industry access standards, encourage social funds to be invested on pension service to promote the development of private nursing institutions and give preferential policy on land using. In addition, the government should improve the salary of nursing assistants and enhance the vocational skills and professional identify .

54 The diagnostic value of growth differentiation factor-15 and N-terminal B-type natriuretic peptide prosoma and high-sensitivity C-reactive protein in patients with acute heart failure

Objective To explore the expression level and diagnostic value of serum growth differentiation factor-15 (GDF-15), N-terminal B-type natriuretic peptide prosoma (NT-proBNP) and high-sensitivity C-reactive protein (CRP) in patients with acute heart failure (AHF) and patients with different severity of AHF. Methods A total of 77 patients with AHF (AHF group) and 60 cases of healthy examined people (control group) from April 2014 to April 2016 in the Second Hospital of Yanbian were selected. The expression level of serum GDF-15 and NT-proBNP were detected by ELISA, the expression of hs-CRP was detected by immunochromatography, routine electrocardiogram and echocardiography were performed to examine the left ventricular ejection fraction (LVEF) and chest X-ray examination. The correlation between GDF-15, NT-proBNP, hs-CRP expression level and the three factors were analyzed. Results Compared with the control group, the expression of GDF-15, NT-proBNP and hs-CRP were significantly increased and the left ventricular ejection fraction (LVEF) was significantly reduced in AHF group, the difference was statistically significant (P < 0.05). The levels of NT-proBNP, hs-CRP among clinical grades Ⅰto Ⅳ had increased trend, the difference was not statistically significant (P > 0.05). GDF-15 and hs-CRP were positively correlated to NT-proBNP, hs-CRP was positivly correlated to NT-proBNP, LVEF and GDF-15 were negatively correlated to NT-proBNP (P < 0.01). The ROC curve analysis showed that the areas under the curve of NT-proBNP, GDF-15 and hs-CRP had statistical significance in diagnosing AHF (P < 0.05), the diagnostic ability was NT-proBNP > GDF-15 > hs-CRP. Conclusion The serum levels of GDF-15, NT-proBNP and hs-CRP are significantly elevated in patients with AHF, and increasd as the clinical severity. The serum GDF-15 can be used as a new index for diagnosis, treatment and estimating prognosis of patients with AHF. GDF-15 combined with the detection of NT-proBNP and hs-CRP has great significance in diagnosing and treating, distinguishing risk stratification and judging prognosis AHF.

59 The effect of Guanxin Shutong Capsules in the treatment of patients with acute myocardial infarction with percutaneous coronary intervention and its effects on oxidative stress and platelet activation

Objective To study the effect of Guanxin Shutong Capsules in the treatment of patients with acute myocardial infarction (AMI) with percutaneous coronary intervention and its effects on oxidative stress and platelet activation. Methods The 80 elderly patients with AMI undergoing percutaneous coronary intervention in Affiliated Hospital of Chengde Medical Univercity from February 2016 to September 2017 were devided into control group (routine treatment) and observation group (Guanxin Shutong Capsules) according to the random number table method, with 40 patients in each group. The clinical efficacy , the levels of superoxide dismutase (SOD), malondialdehyde (MDA) and nitric oxide (NO), and the contents of platelet activation index such as lysosomal membrane glycoprotein (CD63), platelet alpha granule membrane glycoprotein (CD62p) and monocyte platelet aggregates (MPA) preoperative and postoperative in two groups were compared. Results The total effective rate in the observation group was higher than that of the control group, the difference was statistically significant (P < 0.05). After operation, the levels of SOD and NO in the observation group were higher than that of the control group, the level of MDA was lower than the control group, the differences were statistically significant (P < 0.05). After operation, the contents of CD63, CD62p, MAP were lower than before operation in two groups, and the observation group was lower than the control group, the differences were statistically significant (P < 0.05). Conclusion Guanxin Shutong Capsules can significantly improve the clinical efficacy for elderly patients with AMI undergoing percutaneous coronary intervention, inhibit the oxidative stress and platelet activation, and play the myocardial protection role.

63 Correlation analysis between asymmetric dimethylarginine and symmetric dimethylarginine and coronary artery disease of kawasaki disease

Objective To study the three biomarkers of L-arginine, asymmetric dimethylarginine (ADMA) and symmetric dimethylarginine (SDMA) are used to predict the prognosis of children patients with kawasaki disease (KD). Methods From January 2012 to December 2013, 39 children with KD in Department of Pediatric in Taihe Hospital were selected as the case group, venous blood was collected, and at the same time, the plasma of 27 children patients matched with age was collected as control group, which of respiratory infection was accompanied by fever and exclusion of involvement of myocardium. The plasma levels of L-arginine, ADMA and SDMA were measured in the case group and control group. Results The plasma levels of L-arginine, ADMA and SDMA before intravenous immunoglobulin (IVIG) treatment of children in the case group KD1 were lower than those of control group (P < 0.05 or P < 0.01). After IVIG treatment, the L-arginine, ADMA, L-arginine/ADMA ratio (AAR) and arginine methylation composite index (AMI) of different treatment time in case group were compared, the differences were highly statistically significant (P < 0.01). The plasma levels of SDMA of children in case group KD1, case group KD2, case group KD3, case group KD4 were all lower than the control group (P < 0.01), but comparison of before and after IVIG treatment, the difference were not statistically significant (P > 0.05). The AMDA/SMDA ratio (ASR) of children in case group KD2, case group KD3, case group KD4 were all higher than control group and case group KD1 (P < 0.01). In the case group KD1, the L-arginine level of children with coronary artery dilation was higher than that in children without coronary artery dilatation (P < 0.01), while in the case group KD2 and the case group KD3, the children patients with coronary artery dilation was lower than that in children without coronary artery dilatation (P < 0.01). Conclusion The levels of L-arginine, ADMA and SDMA may be related to KD. Low level of L-arginine and ASR of after IVIG treatment are associated with children patients with abnormal coronary artery KD.

67 Analysis of GDF-15 gene polymorphism and acute myocardial infarction

Objective To investigate the relationship between rs1058587 polymorphism of GDF-15 gene and acute myocardial infarction (AMI). Methods From January 2016 to January 2017, 120 patients with AMI of Cardiovascular Medicine Department in the Second Affiliated Hospital of Hainan Medical University were selected as AMI group. In the same period, 120 healthy subjects were selected as health group. The two groups were used by polymerase chain reaction PCR GDF-15 gene rs1058587 locus genotypes, allele frequencies, and explore its relationship with patients with concurrent AMI. Results The genotypes of CC, CG and GG at rs1058587 of GDF-15 gene in AMI group were compared with health group, the difference was statistically significant (P < 0.05). The frequency of G allele of rs1058587 in GDF-15 gene in AMI group was higher than health group (P < 0.05). The results of Logistic multifactor regression analysis showed that the decrease of high-density lipoprotein cholesterol and the rise of Low density lipoprotein cholesterol, serum lipoprotein (a), minim white egg, the allele G at rs1058587 in GDF-15 gene were the independent risk factor for concurrent AMI (P < 0.05). Conclusion Rs1058587 polymorphism of GDF-15 gene is closely related to AMI occurrence in population.

71 Clinical significance of the change of peripheral blood thymidine kinase 1 in childhood acute leukemia

Objective To study the diagnostic value and clinical significance of serum thymidine kinase 1 (TK1) levels in different types of children with acute leukemia (AL). Methods A total of 86 children with AL from the Affiliated Hospital of Guizhou Medical University and Guiyang Children′s Hospital from October 2015 to September 2017 were selected as the study group, they were divided into acute B-cell lymphoblastic leukemia (B-ALL) group (38 cases), acute myeloid leukemia (AML) group (26 cases) and acute T-cell lymphoblastic leukemia (T-ALL) group (22 cases). At the same time, children with acute lymphoblastic leukemia (ALL) were divided into standard risk group (21 cases), intermediate-risk group (19 cases) and high risk group (20 cases), meanwhile 86 healthy children were selected as the control group. Fasting venous blood was collected and serum TK1 levels were measured. Results The levels of serum TK1 in B-ALL group, AML group and T-ALL group were higher than that of control group, the level of serum TK1 in T-ALL group was higher than that of AML group, the level of serum TK1 in AML group was higher than that of B-ALL group, with the statistically significant differences (P < 0.05). The level of serum TK1 In the high-risk group was higher than that of the intermediate-risk group, the level of serum TK1 in the intermediate-risk group was higher than that of the standard risk group, with the statistically significant differences (P < 0.05). Conclusion Detection of the level of serum TK1 has an important clinical reference value in the screening and laboratory diagnosis of AL in children. The level of serum TK1 is related to the clinical risk grade of children with ALL, which can be used as a reference index for its risk classification.

74 Clinical study on combining Edaravone with Shuxuening Injection in treatment of patients with acute cerebral infarction

Objective To observe the clinical efficacy of Edaravone combined with Shuxuening Injection in treatment of Acute Cerebral Infarction. Methods From January 2013 to February 2016, seventy-seven patients with acute ischemic stroke treated in Changping District Hospital of Traditional Chinese Medicine were selected, according to their visit sequences, they were randomly divided into treatment group (n=39) and control group (n=38). Patients in the control group were treated with Shuxuening Injection on the basis of regular treatment, and patients in the treatment group were treated with Edaravone combined with Shuxuening Injection. The neurologic impairment and effects after treatment were observed and compared. Results The defectiveness ratings of nervous functions, the assessment of self-care ability, and the efficacy of the treatment group were noticeably higher than those of control group, the differences were statistically significant (P < 0.05). Compared with before treatment, the plasma viscosity and total blood viscosity of the control group decreased significantly after treatment, and the difference was statistically significant (P < 0.05). There was no significant change in plasma viscosity and total blood viscosity in treatment group before and after treatment (P > 0.05). There was no adverse effect in both groups. Conclusion Shuxuening Injection combined with Edaravone has a significant clinical effect in treating acute cerebral infarction, which can effectively improve nervous function of the patients and their self-care ability.

78 Curative effect of Pramipexole combined with hyperbaric oxygen chamber in the treatment of Parkinson disease combined with dyssomnia

Objective To discuss the curative effect of Pramipexole and hyperbaric oxygen chamber in the treatment of Parkinson disease combined with dyssomnia. Methods The clinical data of 135 patients with Parkinson disease combined with dyssomnia treated in Affiliated Hospital of Yan'an University from December 2014 to December 2016 was analyzed retrospectively. The patients were divided into three groups according to different treatment methods, with 45 cases in each group. The patients in the Pramipexole group were treated with oral Pramipexole Dihydrochloride Tablets, and the patients in the hyperbaric oxygen chamber group were treated with hyperbaric oxygen therapy, the patients in the combination group were treated with hyperbaric oxygen and Pramipexole Dihydrochloride Tablets. All groups were treated for 2 months. The sleep related indices (time in bed, total sleep time, sleep efficiency, sleep onset latency, number of awakenings) and polysomnogram monitoring parameters [sleep time at phase Ⅰ-Ⅱ, slow-wave sleep (SWS), and rapid eye movement sleep (REM) time] after treatment in the three groups were observed and recorded. The scores of Pittsburgh sleep quality index scale (PSQI), Epworth sleepiness scale (ESS), unified Parkinson disease rating scale (UPDRS-Ⅲ), Hoehn-Yahr grading (H-Y grading) of Parkinson disease among the three groups before and after treatment were compared. The conditions of adverse reactions in the three groups were recorded. Results There were no significant differences in the scores of sleep quality related scales among the three groups before treatment (P > 0.05). There were no statistically significant differences in the scores of sleep quality related scales of the Pramipexole group and hyperbaric oxygen chamber group before and after treatment (P > 0.05), while the scores of PSQI, ESS and UPDRS-Ⅲ after treatment in the combination group were significantly lower than those before treatment (P < 0.05), and the scores of PSQI and ESS in the combination group were significantly lower than those of Pramipexole group and hyperbaric oxygen chamber group (P < 0.05), while the scores of UPDRS-Ⅲ and H-Y grading had no significant differences compared with those of Pramipexole group and hyperbaric oxygen chamber group (P > 0.05). After treatment, the time in bed, total sleep time and sleep efficiency in the combination group were significantly higher than those of Pramipexole group and hyperbaric oxygen chamber group (P < 0.05), the number of awakenings was significantly less than those of Pramipexole group and hyperbaric oxygen chamber group (P < 0.05). There were no statistically significant differences of the other sleep indices among the three groups (P > 0.05). No obvious adverse reactions occurred in the three groups after treatment. Conclusion Pramipexole combined with hyperbaric oxygen chamber in the treatment of Parkinson disease combined with dyssomnia has significant clinical effect, which can significantly improve sleep efficiency and quality, it is worthy of clinical promotion.

82 Analysis of antibodies IgG and IgM against TORCH-associated pathogens in serum samples

Objective To provide theoretical guidelines for reasonable application of TORCH assay in aristogenesis and fine rearing through analysis of serum antibody of IgG and IgM against TORCH-associated pathogens (TOX, RV, CMV, HSV1/2) from the pre-pregnant, pregnant patients at early stage and new-born patients. Methods From January of 2014 to December of 2017, 19 533 serum samples were collected from pre-pregnant (pre-pregnant group, 1929 samples), pregnant patients at early stage (pregnant group, 15 555 samples) and new-born patients (new-born group, 2049 samples) in the First People's Hospital of Kunming. ELISA was performed to detect the levels of antibodies IgG and IgM against TORCH-associated pathogens and further determine positive rates of specific antibodies of IgG and IgM. Results ELISA of TORCH antibodies in 19 533 serum samples showed that the positive rate of antibody IgG against any of TORCH-pathogens reached 99.41%. The positive rate of CMV IgG (97.27%) was the highest among four kinds of TORCH-associated pathogens and TOX (3.7%) showed the lowest rate. The positive rate of IgM antibody was 12.8%. The positive rate of HSV1 (6.44%) was the highest and CMV showed the lowest rate (0.39%). Further analysis of the positive rate of IgG by groups showed the more than 99% of IgG positive rate of TORCH-associated pathogens in all groups of prepregnant group, pregnant group at early stage and new born group. The IgM positive rates of TORCH-associated pathogens in pre-pregnant group and pregnant group at early stage, 18.87% and 13.65%, were higher than that in new-born group (0.63%). There were more than 60% of individual serum sample showed the IgG antibodies against 3 TORCH-associated pathogens. However, most of the IgM antibody in individual serum sample was against one of TORCH-associated pathogens. Conclusion The pre-pregnant, pregnant women at early stage and new-born patients are screened for antibodies of IgG and IgM in serum against TORCH-associated pathogens, which is of instructive significance for controlling infections of TORCH pathogens and aristogenesis and fine rearing.

86 Research of DNA quantitative analysis of uterine cavity exfoliated cells in endometrial lesions screening

Objective To investigate the feasibility and application value of DNA quantitative analysis of uterine cavity exfoliated cells in screening endometrial lesions. Methods Five hundred patients with suspected endometrial lesions from January 2014 to May 2016 in the Second Affiliated Hospital of Guiyang Traditional Chinese Medicine College were selected as the research objects, specimens of uterine cavity exfoliated cells were collected, DNA ploidy was analyzed by Feulgen staining and automatic DNA quantitative analysis system, segmented diagnostic curettage was performed at the same time, the segmented diagnostic curettage results were the gold standard, the sensitivity and specificity of DNA quantitative analysis in diagnosis of endometrial lesions was observed. Results Segmental diagnostic curettage detection showed, 39 cases of endometrial carcinoma (78.0%), 98 cases of precancerous lesion (19.6%), 258 cases of endometrial hyperplasia (51.6%), 105 cases of normal endometrium (21.0%). The detection rate of endometrial carcinoma of DNA ploidy analysis of uterine cavity exfoliated cells was similar to pathological examination, the difference was not statistically significant (P > 0.05). DNA ploidy analysis of exfoliated uterine cavity precancerous lesions were similar to the pathological examination, the difference was not statistically significant (P > 0.05). With the pathological biopsy results as the gold standard, the sensitivity, specificity and accuracy of endometrial lesions DNA quantitative analysis were 92.7%, 83.8% and 86.2% respectively. The sensitivity, specificity and accuracy of DNA quantitative analysis in screening endometrial cancer were 94.9%, 67.7% and 69.8% respectively. Conclusion DNA quantitative analysis of endometrial cancer has high screening sensitivity, can be used for early screening of endometrial lesions, has a certain clinical value.

91 Correlation analysis between thyroid function and blood lipid in pregnant women

Objective To investigate the correlation between thyroid function and blood lipid in pregnant women. Methods From April 2015 to April 2016, 250 cases of pregnant women were admitted to the Yancheng City No.1 People′s Hospital were selected, and they were divided into subclinical thyroid function hyperfunction group, hyperthyroidism group and normal group, subclinical hypothyroidism group, hypothyroidism group, 50 cases per group. Radioimmunoassay was used to detect the serum free triiodide (FT3) and free thyroxine (FT4) levels. Immunofluorescence analysis was used to detect the levels of thyrotropin (TSH) in serum. Determination of blood TC and TG levels in each group was taken by enzyme colorimetry. Determination of HDL-C and LDL-C levels was taken by immunosuppressive. The correlation analysis was conducted with FT3, FT4, TSH level and TC, TG, HDL-C level in blood. Results There was a statistical difference in FT3, FT4, TSH and serum lipid between every group (P < 0.05). The TSH level has positively correlated with TC, TG, LDL-C level (P < 0.05), and has no significant correlation with HDL-C (P > 0.05). The FT3 and FT4 levels had significantly negatively correlated with TC, TG and LDL-C levels (P < 0.05), and had no significant correlation with HDL-C level (P > 0.05). Conclusion Thyroid hormone levels in pregnancy are closely related to blood lipids. Hyperthyroidism in pregnancy can lead to levels of TG, TG, and LDL-C decreased.

95 The expression of E-cadherin and Claudin-7 in cervical squamous cell carcinoma and its relationship with clinicopathological features

Objective To investigate the expression of E-cadherin and Claudin-7 in cervical squamous cell carcinoma and its relationship with clinicopathological features. Methods The normal cervical epithelial tissues (control group, n=30), high squamous intraepithelial neoplasias (HSIL group, n=28) and cervical squamous cell carcinomas (CA group, n=80) in the First Affiliated Hospital of Xinjiang Medical University from August 2016 to August 2017 were selected as the research samples. The expression of E-cadherin and Claudin-7 protein in the three groups was detected by the immunohistochemical method. The relationship between them and pathological characteristics of cervical squamous cell carcinoma was analyzed, and the correlation between the expression levels of the two was also analyzed. Results The positive expression rate of E-cadherin protein was significantly higher in the control group than in HSIL group and CA group, and significantly higher in HSIL group than in CA group (P < 0.05). The positive rate of Claudin-7 protein was significantly lower in the control group than in HSIL group and CA group, and significantly lower in HSIL group than in CA group (P < 0.05). The positive expression rate of E-cadherin protein was negatively correlated with the size and lymph node metastasis of cervical squamous cell carcinoma (P < 0.05), and positively correlated with the differentiation degree (P < 0.05). The positive expression rate of Claudin-7 protein was significantly positively correlated with lymph node metastasis (P < 0.05), and negatively correlated with the differentiation degree and clinical stage (P < 0.05). The expression of Claudin-7 protein was negatively correlated with the expression of E-cadherin protein (P < 0.05). Conclusion The positive expression rates of E-cadherin and Claudin-7 protein in cervical squamous cell carcinoma are significantly decreased and increased, respectively. The positive expression rate of them is closely related to the clinicopathological features of cervical squamous cell carcinoma.

99 Efficacy of bioelectrical stimulation combined with pessary in the treatment of over degree Ⅲ pelvic organ prolapse with stress urinary incontinence

Objective To explore the effect of bioelectrical stimulation combined with pessary in the treatment of over degreeⅢ pelvic organ prolapse (POP) with stress urinary incontinence (SUI). Methods Fifty patients of over degree Ⅲ POP with SUI in people′s Hospital of Jiangmen from Jan 2013 to Jan 2017 were divided into mild (15 cases), moderate (21 cases) and severe (14 cases) group according to the severity of urinary incontinence, all cases were treated by bioelectrical stimulation combined pessary. The ICI-Q-SF scores and urodynamic indexs were compared after treatment. Results In the mild and moderate groups, 7 cases were cured (19.4%), 9 cases (25.0%) were significantly improved, 5 cases (13.9%) were ineffective, the total effective rate was 86.1%. The ICI-Q-SF scores and urodynamic indexs were obviously improved after treatment (P < 0.05). But there were no cured and significantly improved cases in the severe group, the effective rate was only 14.3%. Conclusion Bioelectrical stimulation combined with pessary can improve the symptoms of over degree Ⅲ POP with SUI, especially in mild and moderate patients. It can be an alternative treatment scheme to the patients who are inoperable or unwilling to accept the surgery. Urodynamics can accurately assess the effect of the treatment.

104 The efficacy and safety of Trastuzumab combined with TC regimen in the treatment of HER-2 overexpressing breast cancer

Objective To investigate the efficacy and safety of Trastuzumab combined with Docetaxel and Cyclophosphamide chemotherapy in the treatment of HER-2 overexpression breast cancer. Methods From October 2015 to October 2016, 79 cases of breast cancer patients in Cancer Hospital Affiliated to Xinjiang Medical University were selected. The patients were divided into the study group (38 cases) and the control group (41 cases) according to the random number table method. The control group was treated with TC combined chemotherapy before radical mastectomy. The study group was treated with Trastuzumab on the basis of control group. After 18 weeks of treatment, the curative effect and adverse reaction rate of the two groups were compared. The results of HER-2 protein, B7-H1 and IL-2 expression before and after treatment were compared. Results After treatment, the total effective rate (RR) of the study group was  significantly higher than that of the control group (P < 0.05). After treatment, HER-2 overexpression rates of the two groups were significantly reduced than before treatment (P < 0.05), and the overexpression rate of the study group was significantly lower than that of the control group (P < 0.05). The positive expression rates of B7-H1 and IL-2 in the study group after treatment were significantly lower than those before treatment (P < 0.05). Adverse reactions occurred in both groups after treatment had no significant difference (P > 0.05). Conclusion Trastuzumab combined with TC regimen in the treatment of patients with HER-2 overexpression breast cancer can effectively reduce the expression of HER-2 protein, B7-H1 and IL-2, improve the clinical curative effect, which can not increase the risk of medication.

108 Effect of laparoscopic total mesorectal excision on postoperative serum levels of GAS, bFGF and MOT and quality of life in patients with low rectal cancer

Objective To investigate the effect of laparoscopic total mesorectal excision on postoperative serum levels of gastrin (GAS), basic fibroblast growth factor (bFGF) and motilin (MOT) and quality of life in patients with low rectal cancer. Methods Total of 92 cases with low rectal cancer treated in Dongyang People′s Hospital were selected as research objects, and randomly divided into the control group and the observation group, with 46 cases in each group. The observation group was treated with laparoscopic total mesorectal excision, the control group was treated with conventional open surgery. The surgery situation, postoperative recovery of gastrointestinal motility and anal function recovery, improvement of life quality after operation were compared between the two groups. Results The surgical time of the observation group was longer than that of the control group, while the length of incision, intraoperative blood loss, postoperative anal exhaust time, time of taking food and hospitalization time were shorter or less than those of the control group (P < 0.05). The incidence of anastomotic fistula and the total incidence of complications in the observation group were lower than those in the control group (P < 0.05). Three days after surgery, levels of GAS and MOT in the observation group were higher than those in the control group (P < 0.05). Three months after surgery, RMTV in the observation group was higher than that in the control group, while the Wexner score was lower than that in the control group (P < 0.05). Quality of life scores of the observation group were higher than those in the control group at different time points after surgery (P < 0.05). Conclusion Laparoscopic total mesorectal excision has little effect on GAS and MOT in patients with low rectal cancer, and it can improve the quality of life of patients.

112 Meta analysis of incidence of cognitive impairment in patients after general anesthesia and intraspinal anesthesia for lower extremity fractures

Objective To analyze on the domestic and foreign literature on the incidences of cognitive impairment of general anesthesia or intraspinal anesthesia after lower extremity fractures by Meta analysis. Methods The related research reports published on Chinese Wanfang, PubMed, VIP, CNKI, Cochrane and EMbase databases from January 2013 to February 2018 on the incidences of cognitive impairment of general anesthesia or intraspinal anesthesia after lower extremity fractures were selected. Two staff members were screened in accordance with the documentation, exclusion criteria and technical roadmap. The quality of the literature was evaluated by the bias risk assessment tool in the Cochrane manual. Then RevMan 5.1 software was used to analyze the retrieved literature by Meta analysis. Results In this study, 10 domestic and foreign related reports were extracted, including 8 domestic documents and 2 foreign documents. In all of the literature, 925 patients were included, among which 415 cases were treated with general anesthesia, and 510 patients were treated with intraspinal anaesthesia. The results of heterogeneity test showed P = 0.42, I2 = 0. After fixed effect model analysis, it was found that the risk of postoperative cognitive dysfunction after general anesthesia for patients with lower limb fractures was significantly higher than that of patients with lower extremity fracture treated by spinal anesthesia (RR = 1.40, 95%CI: 1.04-1.88, P = 0.04). The results of the bias risk analysis was not found obvious bias. Conclusion The risk of cognitive impairment after general anesthesia in patients with lower extremity fractures is significantly higher than that in spinal canal anesthesia.

116 Application of ring ligation in severe mixed hemorrhoids with rectal mucosal prolapse

Objective To investigate the application of ring ligation in severe mixed hemorrhoids with rectal mucosal prolapse. Methods From October 2015 to September 2017, in Huai′an Hospital of Traditional Chinese Medicine Affiliated to Nanjing University of Chinese Medicine, 88 severe mixed hemorrhoids patients with internal rectal prolapse were selected, they were divided into study group (46 cases) and control group (42 cases). The control group was given Milligan surgery; study group treated with apron ligation treatment. The clinical effect, operative time, postoperative defecation time, hospitalization time, intraoperative blood loss, pain score, analgesic use in two groups were compared, the bleeding, edema, infection, delayed healing, urinary retention, anal stenosis, anal bulge and other complications were recorded and compared, patients' satisfaction after treatment was compared. Results After treatment, the clinical effect of study group was better than control group, the difference was statistically significant (P < 0.01). The operative time, postoperative defecation time, hospitalization time of study group were shorter than control group, the difference were statistically significant (P < 0.01). Intraoperative blood loss of study group was less than control group, the difference was statistically significant (P < 0.01). Postoperative pain scores of two groups reduced, and that of study group was lower than contol group, analgesic use times of study group was lower than control group, the difference were statistically significant (P < 0.01). After treatment, patients in two groups occured bleeding, edema, infection, delayed healing, urinary retention, anal stenosis, anal bulge and other complications, the compltcation rate of study group was lower than control group, the difference was statistically significant (P < 0.01). After treatment, satisfaction of study group was better than control group, the difference was statistically significant (P < 0.01). Conclusion The treatment of severe mixed hemorrhoids with rectal mucosal prolapse with rubber ring ligation can effectively shorten the hospitalization time, reduce pain and incidence of disease and improve patient satisfaction.

120 Clinical relationships between IGF-1,hs-CRP,NT-proBNP and left ventricular hypertrophy in elderly patients with type 2 diabetes

Objective To investigate the clinical relationships between islet growth factor 1 (IGF-1), high sensitivity C reactive protein (hs-CRP) and N-terminal B type natriuretic peptide (NT-proBNP) and left ventricular hypertrophy in elderly patients with type 2 diabetes. Methods From June 2015 to February 2017, 163 elderly patients with type 2 diabetes were selected in the Second Hospital of Xingtai (“our hospital” for short), including 74 patients with left ventricular hypertrophy (hypertrophy group) and 89 patients without any complications (diabetes group). During the same period, 79 healthy persons in our hospital were selected (control group). The left ventricular end diastolic diameter (LVIDd), systolic diameter (LVIDs), end diastolic posterior wall thickness (LVPWD), end diastolic ventricular septal thickness (IVST), left ventricular mass indexes (LVMI) and the levels of serum IGF-1, hs-CRP and NT-proBNP were compared, then the relationships between serum indexes and left ventricular hypertrophy of hypertrophy group were analyzed. Results The LVIDd, LVIDs, LVPWD, IVST, LVMI and the serum levels of hs-CRP and NT-proBNP of the hypertrophy group and diabetes group were significantly higher than those of the control group (P < 0.05), of which the hypertrophy group were significantly higher than those of the diabetes group (P < 0.05). The serum levels of IGF-1 of the hypertrophy group and the control group were significantly higher than those of the diabetes group (P < 0.05), of which the hypertrophy group was significantly higher than that of the control group (P < 0.05). The levels of IGF-1, hs-CRP and NT-proBNP were positively related to LVIDd, LVIDs, LVPWD, IVST and LVMI (P < 0.05). Conclusion The serum levels of IGF-1, hs-CRP and NT-proBNP in left ventricular hypertrophy in elderly patients with type 2 diabetes mellitus are significantly increased, which have closed relationships with left ventricular hypertrophy.

Objective To approach result of treatment cerebral infarction convalescence hemiplegia patients by different acupuncture frequency. Methods The clinical data of 90 cases with cerebral infarction convalescence hemiplegia in Xiaolan People′s Hospital of Zhongshan from April 2014 to April 2017 were retrospectively analyzed, which was to be divided into two groups by different treatment method. Control group  (acupuncture 1 time/d) 45 cases and detection group (acupuncture 2 times/d) 45 cases. Continue treatment of 4 courses were carried out in two groups, the ADL score, nerve function defect degree score, muscular tension score, whole blood viscosity, plasma viscosity and red cell pressure product of two groups before treatment and after treatment were detected, the clinical treatment total effectiveness, adverse reactions rate of two group were detected. Results The ADL score, nerve function defect degree score, muscular tension score, whole blood viscosity, plasma viscosity and red cell pressure product of two groups before treatment had no statistically significant difference  (P > 0.05) . The ADL score of two groups after treatment was higher than before treatment, the nerve function defect degree score, muscular tension score, whole blood viscosity, plasma viscosity and red cell pressure product were lower than before treatment, the ADL score of the detection group after treatment were higher than those of the control group, the nerve function defect degree score, muscular tension score, whole blood viscosity, plasma viscosity and red cell pressure product were lower than control group, the clinical treatment total effectiveness of detection group was higher than those of the control group, the differences were statistical significance  (P < 0.05) . Nausea, diarrhea, constipation, headache rate of two groups had no statistically significant difference  (P > 0.05). Conclusion Acupuncture 2 times/d can improve nerve function and muscular tension of patients with cerebral infarction convalescence hemiplegia, increase living ability, decrease blood viscosity, increase clinical efficacy, it is worth to be used in chinic.

127 Clinical value of thoracic epidural block in treating heart failure of dilated cardiomyopathy

Objective To investigate the clinical value of thoracic epidural blockade for heart failure with dilated cardiomyopathy. Methods From January 2015 to June 2017, 80 cases with heart failure with dilated cardiomyopathy in Deqing Hospital in Huzhou City were selected and divided into control group and observation group, each group with 40 cases. The control group was received routine medical conservative treatment, the observation group were underwent invasive thoracic epidural subjective intrathecal anesthesia and local anesthesia. All patients were observed during hospitalization. All participants were observed at the time point within the course of treatment as a starting point, 3 days after intervention the two group with stress related factors: plasma levels of epinephrine (E) and norepinephrine (NE) were compared. One month after intervention, the echocardiographic parameters of left ventricular diameter and left ventricular ejection fraction changes were compared, one day and one week after intervention, brain natriuretic peptide (BNP) trends were compared also. Results There was no significant difference in plasma E and NE levels between the two groups before intervention (P > 0.05). Three days after intervention, plasma E and NE levels in both groups were significantly lower than those before intervention (P < 0.05). After intervention, the levels of plasma E and NE in the observation group were significantly lower than control group (P < 0.05). Before the intervention, the left ventricular diameter and the left had no statistically significant difference (P > 0.05). After intervention, the left ventricular ejection fraction and left ventricular ejection fraction of echocardiography in observation group were better than before intervention, and the left ventricular diameter and left ventricular ejection fraction of echocardiography in observation group after intervention were better than control group after intervention, the differences were statistically significant (P < 0.05). There was no significant difference in BNP levels between the two groups before intervention (P > 0.05). The levels of BNP in the two groups were lower than those before the intervention, the levels of BNP in observation group were significantly lower than those in control group, the differences were statistically significant (P < 0.05). Conclusion For patients with dilated cardiomyopathy complicated with heart failure, the implementation of thoracic epidural block has important value in reducing stress response, delaying myocardial expansion, improving myocardial oxygen consumption and promoting heart failure recovery.

131 A comparative study of modified Miccoli and traditional surgery in treatment of thyroid gland lobectomy

Objective To compare the effect of modified Miccoli and traditional open surgery in the treatment of thyroid gland lobectomy. Methods From June 2014 to June 2017, in Fushun Central Hospital, 144 patients with benign thyroid diseases were selected, they were divided into control group and observation group, with 72 cases in each group, according to the random number table method, observation group was given modified Miccoli surgery, control group was given traditional surgery. The surgical indexes such as the length of incision, the time of operation, the surgical indexs, VAS and RSI scores postoperative 3 days, VHI score postoperative of 3 months and cosmetic effect in two groups were compared, and the incidence of blood calcium, parathyroid hormone (PTH) before and after operation and the incidence of complications in two groups were compared. Results The postoperative drainage volume in observation group was lower than control group, the hospitalization time was shorter than control group, and the recovery time of cervical movement was earlier than control group, the differences were statistically significant (P < 0.01); compared with control group, the patients in observation group had less flow rate, shorter hospitalization time and faster recovery time in the neck, the differences were statistically significant (P < 0.01). After 3 days of operation, the VAS score observation group was significantly lower than control group, the difference was statistically significant (P < 0.01). But there was no significant difference in RSI score and VHI score between two groups after the operation 3 months. After 3 days of operation, the level of blood calcium and PTH in observation group was significantly higher than control group, the differences were statistically significant (P < 0.01). The incidence of adverse reactions in observation group was significantly lower than control group, the differences were statistically significant (P < 0.01). Conclusion Modified Miccoli is better than traditional operation in the treatment of thyroid lobectomy. It has the advantages of less trauma, quicker recovery and better appearance. It helps maintain the stability of blood calcium and PTH, and has fewer side effects. It is worthy of clinical promotion.

139 Effectiveness evaluation on pharmaceutical intervention of the perioperative antimicrobial drugs use in typeⅠincision in Orthopedic Department

Objective To evaluate the effect on pharmaceutical intervention of the perioperative antimicrobial drugs use in typeⅠincision in Orthopedic Department. Methods Through the DIS system of hospital, electronic medical records of perioperative antibacterial drugs used in typeⅠincision in Sichuan Orthopedic Hospital from January to June 2016 (before intervention, 127 cases in control group) and from January to June 2017 (after intervention, 133 cases in intervention group) were selected. The time of administration, drug varieties, usage and dosage, medication treatment, and so on of perioperative antibacterial drugs used in typeⅠincision in orthopedic department before and after the intervention were retrospectively analyzed. Results After intervention, the reasonable proportion of the time of administration of perioperative antibacterial drugs varieties used in intervention group was significantly increased to 84.2%, and it compared with the control group (51.2%), the difference was highly statistically significant (χ2=32.81, P=0.000). The rational proportion of perioperative antibacterial drugs used in intervention group was compared with the control group, the difference was highly statistically significant (χ2=46.27, P=0.000). Administration route of antimicrobial drugs before and after the intervention were intravenous drip, and the two groups were compared on the route of administration, the difference was not statistically significant (P > 0.05), but the dosage and frequency of medication in intervention group were higher than those of the control group (χ2=23.11, P=0.000, χ2=36.32, P=0.000). The antibacterial drug of treatment in the operation of < 24 h in the two groups were compared, the difference was not statistically significant (P > 0.05). The medication treatment in the operation of 24-48 h in the intervention group was higher than the control group (χ2=23.78, P=0.000). The medication treatment in the operation of > 48 h in the intervention group was lower than the control group (χ2=18.42, P=0.000). Conclusion The effect of pharmaceutical intervention of the perioperative antimicrobial drugs use in typeⅠincision in Orthopedic Department is definite, and can effectively improve the rational utility ratio of antibacterial drugs, reduce the occurrence of non-standard drugs use, which is worth promoting.

143 The efficacy of Endostar combined with compound Kushen Injection in the treatment of advanced non-small cell lung cancer malignant pleural effusion

Objective To evaluate the efficacy and adverse reactions of recombinant human endostatin (Endostar) combined with compound Kushen Injection in the treatment of advanced non-small cell lung cancer (NSCLC) malignant pleural effusion. Methods Sixty-eight cases of advanced NSCLC patients with malignant pleural effusion from June 2015 to June 2017 in People's Hospital of Wuhan University were randomly divided into treatment group and control group, with 34 patients in each group. In the control group, 30 mg of Endostar was injected into the thoracic cavity and repeated once a week later. On the basis of this, the treatment group was diluted 20 mL of compound Kushen Injection with 250 mL of Sodium Chloride Injection and intravenously instilled once a day, 14 days was used as a course of the treatment. After one month of treatment, the clinical efficacy and adverse reactions were observed. Results The objective remission rate and disease control rate in the treatment group were significantly higher than the control group, the differences were statistically significant (P < 0.05). Adverse reactions of the two groups were mainly nausea, vomiting, fatigue and myelosuppression. The main adverse reactions were grade Ⅰ to Ⅱ, the differences were not statistically significant (P > 0.05). Conclusion The efficacy of Endostar combined with compound Kushen Injection in the treatment of advanced NSCLC malignant pleural effusion was significant, and little adverse reactions, with a relatively positive clinical value.

147 Qualitative research on cognition and attitude of palliative care nurses to palliative care

Objective To deeply understand the cognition and attitude of the nurses from palliative department toward palliative care. Methods From September to October 2017, with phenomenological method of qualitative research as the guidance, and with purposive sampling method and semi-structured interviews adopted, 10 nurses from palliative department in a third grade class-A tumor hospital in Xinjiang Uygur Autonomous Region were interviewed and relevant materials were collected and analyzed. Results Four themes were extracted from analyzing the interview materials: ①the cognition to the concept of palliative treatment; ②the implementation of palliative care; ③the intervention to family members; ④the problems faced in the implementation of palliative care. Conclusion Palliative care nurses have a positive attitude towards the implementation of palliative care, and the promotion of palliative care concept should be strengthened.

Objective To observe the influences of ear massage on postoperative nausea and vomiting, cognitive function and sleep quality in elderly patients treated with radical operation of thyroid cancer under endoscope. Methods From January 2016 to January 2017, 140 patients with thyroid cancer in Traditional Chinese Medicine Hospital Affiliated to Xinjiang Medical University were collected as observational subjects, and all patients were treated with radical operation of thyroid cancer under endoscope, their clinical materials were retrospectively analyzed, according to the nursing method, the patients were divided into the control group (62 cases) and the observation group (78 cases). The control group was given conventional nursing, the observation group was given ear massage based on the control group. Mini mental state examination (MMSE) and Pittsburgh sleep quality index (PSQI) were used to assess cognitive function and sleep quality. The incidences of postoperative nausea and vomiting, scores of MMSE, PSQI in the two groups were compared. Results Ccompared with the control group, the observation group had lower total incidence of nausea and vomiting, higher MMSE score 5 d after operation and lower PSQI scores 5 d after operation, the differences were statistically significant (P < 0.05 or P < 0.01). Conclusion Ear massage has well application effect in elderly patients treated with radical operation of thyroid cancer under endoscope. And it can effectively reduce incidence of nausea and vomiting, alleviate cognitive function injury and improve sleep quality.

154 Effects of cognitive behavioral therapy on sleep quality in patients with cancer on chemotherapy

Objective To determine effects of cognitive behavioral therapy on sleep quality in cancer patients treated with chemotherapy. Methods From January 2010 to January 2015, in Affiliated Hospital of Yan′an University, 88 cancer patients received chemotherapy were selected, they were divided into observation group (43 cases) and control group (45 cases). The control group was given routine treatment, the  observation group was given cognitive behavioral therapy based on the control group. The Pittsburgh Sleep Quality Index (PSQI) and Zung′s self-rating anxiety scale (SAS), Zung′s self-rating depression scale (SDS) were used to assess the sleep quality and psychological status after intervention 1 month and 3 months. Results After 3 months intervention, SAS scores ,SDS scores and PSQI of two groups were significantly lower than before the intervention, the differences were statistically significant (P < 0.05); SAS scores, SDS scores and PSQI of the observation group were lower than control group, the differences were statistically significant (P < 0.05). Conclusion Cognitive behavioral therapy significantly reduces the levels of depression, anxiety and the severity of insomnia in cancer patients treated with chemotherapy.

158 Application effect of patient-oriented cognitive rehabilitation training in elderly patients with vascular dementia

Objective To investigate the application effect of patients-oriented cognitive rehabilitation training in elderly patients with vascular dementia. Methods From November 2014 to July 2016, 63 cases of hospitalized elderly patients with vascular dementia in the Second People′s Hospital of Chengdu were selected as the research object and divided into the training group (32 cases) and the control group (31 cases) randomly according to the admission number. The control group was given routine nursing care, while the training group was given patient-oriented cognitive rehabilitation training on the basis of control group and the training time was 8 weeks. The functional independence scale (FIM), computer neuropsychological test (CNT) and mini mental state examination scale (MMSE) were used to evaluate the activities of daily living and cognitive function before and after training. Results Before training, there were no statistically significant differences in the scores of FIM and MMSE between the two groups (P > 0.05). After training, the scores above were all higher than those of before training, and the scores in the training group were higher than those of control group, with statistically significant differences (P < 0.05). Before training, there were no statistically significant differences in the scores of each item in CNT between the two groups (P > 0.05). After training, except the language learning test (LLT), the testing results of digit span test (DST), visual span test (VST), visual learning test (VLT) and visual continuous performance test (VCPT), auditory controlled continuous performance test (ACCPT) in the training group were all significantly superior to the control group, and the sores in the training group were all superior to the control group, with statistically significant differences (P < 0.05). Within the groups, after training the sores of DST, VST, VLT, VCPT and ACCPT in the training group were all significantly superior to those of before training; but there were no statistically significant differences in the control group (P > 0.05). Conclusion During the process of caring elderly patients with vascular dementia, implementation of patients-oriented cognitive rehabilitation training may help to improve their activities of daily living and cognitive function, which is worthy of promotion and application in clinical.

162 The application effect of quality control circle in clinical teacher training

Objective To explore the practice effect of quality control circle in clinical teacher training. Methods From June 2016 to June 2017, quality control circle was applied to train the clinical teachers in the Second Affiliated Hospital of Xinjiang Medical University, including establishing of quality control circle, selecting the theme, drawing up the program of activities, grasping the status quo, setting goals and implementing countermeasures. The teaching ability of clinical teachers and teaching effect were evaluated, and the practice nurses′ satisfaction of the clinical teaching before and after training were investigated. Results After training, the scores of clinical teaching theory, clinical nursing knowledge, clinical skills and teaching ability were higher than those of before training, with statistically significant differences (P < 0.05). After clinical teachers trained by quality control circle, the scores of theory and operation achievement in practice nurses were higher than those of before training, with statistically significant differences (P < 0.05). The satisfaction on clinical nursing teaching in practice nurses was higher than that of before training, with statistically significant differences (P < 0.05). Conclusion The application of quality control circle in the training of clinical teachers can effectively improve the teachers′ comprehensive quality and teaching ability, improve the theory and operation achievement of practice nurses, and can improve the clinical teaching satisfaction. It is worthy of clinical promtion and application.

166 Nursing effect of early head mild hypothermia treatment on patients with acute ischemic stroke

Objective To explore the effect of early head mild hypothermia treatment on patients with acute ischemic stroke. Methods From January 2016 to January 2017, 100 patients with acute ischemic stroke admitted to the Stroke Center, the First People's Hospital of Lianyungang were selected as the research subjects and divided into the control group and the observation group according to the random number table, with 50 cases in each group. The control group was given systemic mild hypothermia treatment after 6 hours of adimitting to the hospital, while the observation group was given the mild treatment of head ice cap sub-hypothermia immediately after the emergency CT diagnosis on the basis of control group. The scores of national instiutes of health stroke scale (NIHSS), modified rankin scale (mRS) and the incidence of complications were compared between the two groups. Results After treatment, the scores of NIHSS, mRS and the incidence of complications in the observation group were significantly lower than those of the control group, with statistically significant differences (P < 0.05). Conclusion Early mild hypothermia treatment can significantly improve the neurological function, reduce the occurrence of adverse events in patients with acute ischemic stroke. It is worthy of clinical promotion and application.

ALS patients always occure respiratory muscle weakness, acute respiratory failure and respiratory muscle paralysis caused by pulmonary infection are the most common cause of death in ALS patients. It is very important for ALS patients to be evaluated regularly and timely hospitalized assessment. This article reviews the respiratory support process of an ALS patients with acute type Ⅱ respiratory failure. After the patient admitted to hospital, the blood gas analysis suggested that type Ⅱ respiratory failure, according to the patient's breathing state, blood gas analysis results and personal wishes, he was treated with non-invasive ventilation, tracheal intubation assisted ventilation and tracheotomy after transferred into the neurological ICU by sequence, and given adequate nutritional support, Antibiotic treatment of infection, expectorant and other symptomatic treatment care at the same time. After the systematic treatment, the patient's condition became stable and transferred to long-term carefacility. Generally, in patients with auxiliary ventilation indications, choosing the appropriate respiratory support measures and regular adjustments according to the disease and personal wishes can help improve the patients' respiratory function.

173 Factors of hospital delay of acute myocardial infarction with percutaneous coronary intervention

Objective To investigate the factors of hospital delay of acute myocardial infarction with percutaneous coronary intervention. Methods Restropective analysis was applied to 324 STEMI cases with PCI from emergency in Beijing Tiantan Hospital from January 2015 to August 2017. Univariate analysis was used to compare group A (DTB≤90min, 198 cases) and group B (DTB>90 min, 126 cases). Logistic multivariate analysis was adopted to explore related factors. Results Factors of hospital delay were as follows: low level of education (OR = 1.761, 95%CI: 1.071-2.894, P < 0.05), mode of transport to the hospital without EMS (OR = 1.765, 95%CI: 1.087-2.867, P < 0.05),  PCI on work-day (OR = 1.936, 95%CI: 1.113-3.369, P < 0.05) and history of digestive diseases (OR = 1.917, 95% CI: 1.085-3.387, P < 0.05). There were some factors that could shorten hospital delay such as initial diagnosis with STEMI (OR = 0.339, 95%CI: 0.207-0.556, P < 0.01) and pre-hospital delay within 120 min (OR = 0.578, 95%CI: 0.353-0.947, P < 0.05). Conclusion It should offer multiform health education to people in order to increase awareness rate and degree of recogniton. Through improving emergency system, enhancing collaboration pre-hospital and in-hospital and promoting more people to use EMS, pre-hospital delay can be shorten. More cultivation on doctors and nurses should be organized to improve ability of triage and prognosis. Optimization of emergency green channel of PCI can promote department cooperation and increase efficiency of DSA to reduce hospital delay.

178 The characteristics of clinical distribution and surveillance of susceptibility for 156 strains of carbapenem-resistant Klebsiella pneumoniae

Objective To investigate the distribution and susceptibility of carbapenem-resistant Klebsiella pneumoniae (CRKP) isolated from the First People′s Hospital of Shangqiu. Methods CRKP was collected from January 2014 to July 2017 and the results were analyzed, producing of carbapenemase and metallo-beta-lactamases were confirmed using Kirby-Bauer method. Results A total of 156 CRKP were collected and mainly from ICU (73.7%,115/156) and respiratory specimens (76.3%,119/156). The results of antimicrobial susceptibility testing showed that the sensitivity of polymyxin, tigecyline, minocycline and tetracycline were 100.0%, 92.8%, 88.5% and 64.1% respectively. The sensitive rates of methoxybenzidine-sulfamethoxazole, chloramphenicol amikacin, and levofloxacin were 39.7%, 30.1%, 29.5% and 23.1% respectively. It was serious resistant to beta-lactam, cephalosporin and its compound preparations. 78.2% CRKP produced carbapenemase (122/156) and 4 strains of metalloenzyme were isolated. Conclusion The patients from the ICU, neurology and geriatrics ward with respiratory tract infection should be importance of strengthen infection control in our hospital. To produce carbapenemase is the main method of local CRKP resistances. The situation of antibiotic resistance of CRKP is very serious except polymyxin and tetracycline species, so polymyxin, tigecyline and minocycline can be used in clinical practice of CRKP infection.
