DOI: 10.20047/j.issn1673-7210.2023.33.01
Objective To investigate the role of six-segment transmembrane epithelial antigen of prostate 4 (STEAP4) in the development of prostate cancer in the inflammatory microenvironment induced by lipopolysaccharide (LPS). Methods The inflammatory environment of prostate cancer cells PC3 and VCaP was induced by LPS. PC3 and VCaP cells were divided into control group, LPS treated group (PC3 and VCaP cells were exposed to 1 μg/ml LPS), transfection control group(LPS+transfection si-con) and transfection silence group (LPS+transfection si-STEAP4). The STEAP4 level was detected 24 h after transfection by Western blot. The expression levels of cytokines interleukin (IL)-6, IL-8 and tumor necrosis factor-α(TNF-α) were detected by enzyme-linked immunosorbent assay. Cell proliferation was detected by CCK-8 and EdU staining. The expression level of cyclic guanosine monophosphate(cGMP)-cGMP-dependent protein kinase(PKG) signaling pathway related proteins was detected by Western blot. Results STEAP4 protein expression level of PC3 and VCaP cells in LPS treated group was significantly higher than that in control group (P<0.05). There was no significant difference in the expression level of STEAP4 protein in PC3 and VCaP cells in LPS treated group compared with transfection control group (P>0.05). The expression level of STEAP4 protein in PC3 and VCaP cells in transfection silence group was significantly lower than that in transfection control group (P<0.05). The expression levels of IL-6, IL-8, and TNF-α in PC3 and VCaP cells in LPS treated group were significantly higher than those in control group (P<0.05). There were no significant differences in the expression levels of IL-6, IL-8, and TNF-α in PC3 and VCaP cells between LPS treated group and transfection control group (P>0.05). The expression levels of IL-6, IL-8, and TNF-α in PC3 and VCaP cells in transfection silence group were significantly lower than those in transfection control group (P<0.05). The proliferation activity of PC3 and VCaP cells in LPS treated group was significantly higher than that in control group at 24, 48, and 72 h (P<0.05). There was no significant difference in the proliferation activity of PC3 and VCaP cells at 24, 48, and 72 h between LPS treated group and transfection control group (P>0.05). The proliferation activity of PC3 and VCaP cells in transfection silence group was lower than that in transfection control group at 24, 48, and 72 h (P<0.05). The proliferation activity of PC3 and VCaP cells in LPS treated group was significantly higher than that in control group (P<0.05). There was no significant difference in the proliferation activity of PC3 and VCaP cells between LPS treated group and transfection control group (P>0.05). The proliferation activity of PC3 and VCaP cells in transfection silence group was significantly lower than that in transfection control group (P<0.05). 56 Research advances in early markers for the diagnosis of acute kidney injury
HAN Yunbiao1 ZHAO Yiwen2 XU Libin1
DOI: 10.20047/j.issn1673-7210.2023.33.11
Acute kidney injury (AKI) is an acute and critical illness in the department of nephrology. Although most physicians have improved their understanding of AKI, there is still a high missed diagnosis rate and mortality rate. Some patients may experience chronic changes. Creatinine and urine volume is the main diagnostic indicator, which is single and hysteresis, lacks the specificity of renal tubular injury, and is easily disturbed by many factors. It is difficult to evaluate AKI early, but with the discovery of some early markers, the early diagnosis and treatment of AKI is possible. This paper summarizes some of the new diagnostic markers, describes the possible pathophysiological processes of different markers in AKI, expounds the feasibility of predicting AKI with some markers, and proposes some checking points for AKI treatment, which will provide guidance for the diagnosis and treatment of AKI in the future.

Recobinant isoleucyl-tRNA synthetase (IARS) catalyzes isoleucine and specific tRNA to bind and regulate protein synthesis, and also regulates intracellular signaling pathways and participates in intracellular metabolic processes. IARS gene variation is very rare in animals and humans. The deficiency of IARS caused by IARS mutation is a rare autosomal recessive genetic disease, resulting in severe developmental delay, impaired intellectual development, hypotonia, and abnormal liver function. And the specific amino acid therapy after IARS gene mutation has also become a current research hotspot. This article reviews the research progress in this field, with a view to summarizing the phenotypes associated with IARS gene variation, clarifying the association between genotype and phenotype, and the possible pathogenesis, and providing references for the etiology research.

130 Professor Zhang Mianzhi’s clinical experience in treating proteinuria based on the theory of “kidney deficiency and blood stasis”
CHEN Shu1 DONG Ao1 ZHANG Mianzhi2
DOI: 10.20047/j.issn1673-7210.2023.33.29
Proteinuria as one of the common clinical manifestations of chronic kidney disease, Chinese medicine treatment of proteinuria can avoid the western medicine treatment brought by the toxic side effects, unclear curative effect, and other problems. Based on this, Professor Zhang Mianzhi uses the theory of “kidney deficiency and blood stasis” to treat proteinuria. Professor Zhang Mianzhi believes that the etiology and pathogenesis of proteinuria are mainly classified as “kidney deficiency and blood stasis”, and spleen deficiency, dampness turbidity, wind evil, and heat toxicity are also key factors leading to the occurrence and development of the disease. Therefore, he advocates that the therapeutic principle of “tonifying kidney and promoting blood circulation” should be carried out in the whole process of disease treatment, and remarkable therapeutic effects have been achieved. Based on the theory of “kidney deficiency and blood stasis”, this article explains the clinical experience of Professor Mianzhi in the treatment of proteinuria, and provides a case for discussion.

Kaixin Powder is a commonly used prescription for traditional Chinese Medicine clinial treatment of “forgettery”. In the process of application by later clinical practitioners, related prescriptions had been evolved. Compared with Kaixin Powder, the theory of related prescriptions are both “righteous” and “innovative”. The article inspects the name of Kaixin Powder and the source of it, discusses the feature of Kaixin Powder and its related prescriptions, and clarifies the change of the treatment scope. As a result, Kaixin Powder comes from the book of Jiyan Fang wrote by an ancient doctor named Yao Sengyuan. It is believed that the name of Kaixin Powder indicates the functional regulation of opening the heart and regulating the kidney. In the process of spreading, this formula has derived seven main types of prescriptions, including Dingzhi Pills, Lingren Buwang Fang, Yuanzhi Powder, Xiaodingzhi Pills, Yuanzhi Pills, Jiawei Dingzhi Pills, and Changpu Yizhi Pills. Kaixin Powder and its related prescriptions follow the theory of compatibility of sweet, bitter, and pungent, and determine the dosage of the main medicine according to the characteristics of the syndrome, and the method of administration according to the characteristics of the dosage form. According to the theory of this group of prescription, the main treatment syndromes of Kaixin Powder and its related prescriptions have gone through a process from simple “forgettery” to abnormal mental state, and then to psychosomatic diseases, which has an enlightening effect on exploring and expanding the clinical application scope of Kaixin Powder.

138 Professor Chang Jingling’s experience in the treatment of post-stroke aphasia based on the theory of “brain-kidney-marrow”
WANG Haifang XU Minjie CHANG Jingling
DOI: 10.20047/j.issn1673-7210.2023.33.31
Post-stroke aphasia is one of the most common complications of stroke, and one of the causes of long-term deficits in speech disorders, which seriously affects patients’ quality of daily life. At present, it is mainly treated with speech rehabilitation therapy combined with conventional western medicine treatment in stroke medicine, which the clinical efficacy varies greatly among individuals. Through years of clinical observation and practice, Professor Chang Jingling concluded that post-stroke aphasia should be treated with “brain-kidney-marrow” as the main line of syndrome differentiation and treatment, and overall syndrome differentiation and treatment of pathological factors such as wind, fire, and phlegm. The theory is based on strengthening vital qi and dispersing blood stasis. The syndrome differentiation adopts the methods of clearing heat and promoting blood circulation, soothing the liver and extinguishing wind, resolving phlegm and unblocking collaterals, awakening the spirit and opening the body, and treating both the tip and root, combining tonifying and supplementing, demonstrating the dialectical thinking advantages of “integration of body and spirit” for traditional Chinese medicine.

153 Professor Yuan Bin’s experience in treating childrens’ new-long cough from“wind-phlegm”
CAO Bingxue1 YUAN Bin2
DOI: 10.20047/j.issn1673-7210.2023.33.34
Professor Yuan Bin inherited the teacher’s theory and made some innovations, on the basis of Professor Wang Shouchuan’s theory of hidden wind, the concept of wind-phlegm cough is put forward. Professor Yuan describes the specific symptoms of this cough. He believes that the key to the pathogenesis is the retention of wind-phlegm in the lung and the imbalance in lung qi. More phlegm and blood in the stasis mutual knot day after day. After consulting ancient and modern literature, Professor Yuan wrote new prescriptions to restore lung qi by reducing phlegm and wind with ephedra and pinellia tuber. Treatment is supplemented with activating the spleen and regulating the liver to coordinate the five internal organs and improve the condition. He attached great importance to the relationship between the lung and the orifices. A distinction between the nose and pharynx should be made, and treatment of organs and orifices should be performed at the same time for internal and external patency. He is skilled in flexible methods with a combination of traditional Chinese medicine and western medicine, though mainly in traditional Chinese medicine. Diagnosis and treatment involve both Chinese and western methods. Professor Yuan bin often attaches importance to the supplementary examination of modern medicine and often combines syndrome differentiation with disease differentiation when diagnosing disease. Professor Yuan Bin’s treatment of the disease is mainly based on traditional Chinese medicine, supplemented by western medicine, and has repeatedly achieved good results.

DOI: 10.20047/j.issn1673-7210.2023.33.42
Pulmonary nodules are the main manifestation of early lung cancer, and modern medicine has formed its standardized management and intervention system, but there are still problems such as lack of intervention measures during the follow-up period. Traditional Chinese medicine has advantages in the diagnosis and treatment of pulmonary nodules. This paper proposes to establish a management and intervention system for pulmonary nodules with Traditional Chinese medicine characteristics, apply traditional Chinese medicine to the entire diagnosis and treatment of pulmonary nodules, form a screening model combining imaging and constitution, construct a malignant risk prediction model with the participation of traditional Chinese medicine syndromes, adopt interventions combining the identification of constitution and syndromes as well as the internal and external treatments in the follow-up period, and set up a comprehensive platform for the networked management and intervention of traditional Chinese medicine in pulmonary nodules. Also, limitations such as the lack of standardization of the traditional Chinese medicine management and intervention protocol in pulmonary nodules, the gaps in the study of the time-pharmacodynamic effect, and the imperfect evaluation of the therapeutic efficacy are discussed.
