Abstract Objective To explore the effects of group-based recreation activities on the improvement of cognitive function, daily living ability, and quality of life in patients with post-stroke cognitive impairment (PSCI). Methods A total of 80 subjects meeting PSCI standards in Rehabilitation Center of Tongde Hospital of Zhejiang Province from June 2021 to May 2023 were selected, and the group was divided into control group and treatment group with 40 cases in each group according to the ratio of 1∶1 by block randomization. The control group received traditional cognitive rehabilitation training, and the treatment group received group-based recreation activities training on the basis of the control group. They were assessed by the mini mental state examination (MMSE), the Montreal cognitive assessment (MoCA), the Barther index (BI) and the World Health Organization quality of life scale (WHO-QoL) before treatment, the occurrence of adverse events in the two groups during the intervention period was analyzed. Results The overall analysis found that there were statistically significant differences in time and interaction between the two groups (P<0.05). In intra-group comparison, MMSE and MoCA scores were compared between the two groups at each time point, and the differences were statistically significant (P<0.05). Before treatment, there were no significant differences in MMSE and MoCA scores between the two groups (P>0.05). After treatment and at follow-up, MMSE scores in the treatment group were higher than those in the control group, and the differences were statistically significant (P<0.05). Overall analysis found that there were statistically significant differences in the comparison of inter-group, time and interaction between the two groups (P<0.05). Intra-group comparison and comparison of BI and WHOQoL scores at each time point between the two groups showed statistically significant differences (P<0.05). Before treatment, there were no significant differences in BI and WHO-QoL scores between the two groups (P>0.05). After treatment and at follow-up, BI and WHOQoL-BREF scores in the treatment group were higher than those in the control group (P<0.05). No adverse events were reported during the study period in either group. Conclusion Group-based recreation activities can effectively improve cognitive function, daily activity ability, and quality of life in PSCI patients.
