Analysis on the medication rule of Professor Gao Ronglin in the treatment of insomnia based on data mining

Objective To analyze and summarize the medication rule of Professor Gao Ronglin in the treatment of insomnia based on data mining technology. Methods Medical records of insomnia patients in the outpatient department of Professor Gao Ronglin of Guang ’anmen Hospital, China Academy of Chinese Medical Sciences from October 2020 to October 2022 were selected. Microsoft Excel 2016 and Rstudio 4.2.1 statistical software were used to make statistics on the frequency, four natures, five flavours, channel tropism, and efficacy of drugs. Association rules and cluster analysis were performed for high-frequency drugs, and complex network visualization was performed using VOSviewer software. Results A total of 146 kinds of traditional Chinese medicine were involved in 358 prescriptions, and the cumulative use frequency was 4 947. Twenty kinds of traditional Chinese medicine, such as Ziziphi Spinosae Semen, Paeoniae Radix Alba, Polygalae Radix, and so on, were used more than 90 times. The four nature of drugs were mainly flat, warm, and cold, the five flavour of drugs were mainly sweet, bitter, and pungent, and the channel tropism of drugs were mainly liver, heart, and spleen. The most frequently used drugs were tonifying deficiency drugs and tranquillization drugs. The results of cluster analysis showed that the drug combinations with the highest support and frequency were Polygalae Radix-Ziziphi Spinosae Semen, followed by Moutan Cortex-Ziziphi Spinosae Semen, Coptidis Rhizoma-Ziziphi Spinosae Semen, and the drug combinations with the highest confidence were Cinnamomi Cortex-Ziziphi Spinosae Semen, Coptidis Rhizoma, Cinnamomi Cortex-Ziziphi Spinosae Semen. Cluster analysis could be divided into four categories. Conclusion Professor Gao Ronglin treats insomnia from liver, heart, and spleen, pays attention to treating the heart and regulating the spirit, regulating the spleen and stomach, and commonly uses Coptidis Rhizoma, Cinnamomi Cortex, and Ziziphi Spinosae Semen couplet medicines concerted application to communicate the mind, tonify the kidney and warm the yang.

Study on the mechanism of medicinal bath of Yaoyi Chumen Decoction in the treatment of rats with knee osteoarthritis based on p38MAPK pathway

Objective To investigate the effect and mechanism of medicinal bath of Yaoyi Chumen Decoction in the treatment of knee osteoarthritis (KOA) model rats based on p38 mitogen-activated protein kinase (p38MAPK) pathway. Methods Ten of 60 8-week-old male SPF SD rats (weight 200-220 g) were randomly selected as blank group, 0.1 ml of physiological saline was injected into the right knee cavity of the blank group on the 1st and 4th days of the experiment. The remaining rats were injected with a mixed emulsion of 0.1 ml type Ⅱ collagenic proteinase and complete Freund’s adjuvant (mass concentration was 0.5 mg/ml). On the 7th day, the modeling results were evaluated according to the knee joint conditions of the rats, 50 rats successfully modeled were divided into model group, Diclofenac Sodium group, and Chumen Decoction high, medium, low dose groups with ten rats in each group according to random number table method. On the 8th day of the experiment, the Chumen Decoction high, medium, low dose groups were given medicinal bath intervention (the concentration was 49.00, 24.50, 12.25 g/L, respectively), the model group was given warm water bath, the Diclofenac Sodium group was given 0.01 g of 1% Diclofenac Sodium Gel at the modeling site, and the blank group was fed routinely, the intervention lasted for four weeks in each group. The general condition of each group was observed during the intervention. After intervention, the anatomy of knee joint in each group was observed, the diameter of knee joint was measured, and the histological changes of knee joint cartilage were observed by hematoxylin-eosin (HE) staining. The serum levels of interleukin-1β(IL-1β), IL-6, and IL-10 were detected by enzyme-linked immunosorbent assay. Immunohistochemical staining was used to detect the location and expression levels of p38MAPK, phosphorylated p38MAPK (p-p38MAPK), and matrix metalloproteinase-13 (MMP-13) in cartilage tissue of each group. Results During the drug intervention, the general condition of the blank group was normal, the model group limp during forced exercise with knee flexion and extension dysfunction, the flexion and extension function of knee joint and gait of Chumen Decoction dose groups were gradually recovered, and the lameness of both feet was reduced. After the intervention, the knee joint anatomy of the blank group showed smooth and translucent synovium, transparent, and smooth cartilage, and complete articular surface, HE staining showed that the chondrocytes were arranged regularly and clearly. The knee joint anatomy of the model group showed synovial thickening and erosion, cartilage surface roughness, and wear, and gray color, HE staining showed that cartilage matrix was reduced or even disappeared, chondrocytes were reduced in large numbers, and the shape was irregular. The knee joint anatomy and HE staining showed that the knee joint cartilage of Chumen Decoction dose groups were improved to varying degrees. The knee diameter of model group was higher than that of blank group, the expressions of serum IL-1β, IL-6, and cartilage p38MAPK, p-p38MAPK, MMP-13 were higher than those of blank group, and serum IL-10 was lower than that of control group (P<0.05). The knee diameter of each Chumen Decoction dose groups were lower than that of model group, the serum IL-10 was higher than that of model group, and the expressions of cartilage p-p38MAPK and MMP-13 were lower than those of model group (P<0.05). Serum IL-1β, IL-6, and cartilage p38 MAPK in Chumen Decoction high and medium dose groups were lower than those in model group and Chumen Decoction low dose group, and serum IL-10 was higher than that in Chumen Decoction low dose group (P<0.05). The knee diameter of Chumen Decoction high dose group was lower than those of Chumen Decoction low and medium dose groups, the expressions of cartilage p-p38MAPK and MMP-13 were lower than those of Chumen Decoction low dose group, the serum IL-10 was higher than that of Chumen Decoction low dose group, and the expression of cartilage MMP-13 was lower than that of Chumen Decoction medium group (P<0.05). Conclusion Yaoyi Chumen Decoction may reduce the inflammatory response and improve cartilage degeneration in rats with KOA through targeted inhibition of p38MAPK pathway expression.

Relationship between serum levels of high motility protein B1, ubiquitin carboxy-terminal hydrolase L1 and poor prognosis of neurological function after cardiopulmonary resuscitation

Objective To investigate the relationship between serum high motility protein B1 (HMGB1), ubiquitin carboxy-terminal hydrolase L1(UCH-L1) and poor prognosis of neurological function after cardiopulmonary resuscitation. Methods A total of 175 patients with spontaneous circulation recovery after cardiopulmonary resuscitation after cardiac arrest admitted to the First Affiliated Hospital of Hainan Medical College from January 2019 to October 2022 were selected as the study objects. The patients were divided into poor prognosis group (75 cases) and good prognosis group (100 cases). Serum HMGB1 and UCH-L1 levels and clinical data (gender, age, body mass index, mechanical ventilation time, smoking history, drinking history, history of previous underlying diseases, cause of cardiac arrest, recovery time of autonomic circulation, pre-hospital emergency time, kidney replacement therapy, Glasgow coma scale (GCS) score, acute physiology and chronic health Ⅱ [APACHEⅡ] score) were compared between the two groups. Multivariate logistic regression was used to analyze the factors affecting the prognosis of neurological function, and receiver operating characteristic (ROC) curve was used to analyze the predictive value of serum HMGB1 and UCH-L1 for poor prognosis of neurological function. Results APACHE Ⅱ score, mechanical ventilation time, and serum HMGB1 and UCH-L1 in the poor prognosis group were higher than those in the good prognosis group, while GCS score was lower than that in the good prognosis group, with statistical significances (P<0.05). Logistic multivariate regression analysis showed that HMGB1, UCH-L1, and APACHEⅡ scores were independent risk factors for poor prognosis of neurological function after cardiopulmonary resuscitation (OR>1, P<0.05). ROC curve analysis showed that HMGB1 combined with UCH-L1 was more effective in predicting poor neurological outcomes than HMGB1 and UCH-L1 alone (P<0.05). Conclusion Serum HMGB1 and UCH-L1 levels are significantly increased in patients with poor neurological prognosis after cardiopulmonary resuscitation, which are independent risk factors for poor neurological prognosis. Combined detection is helpful for early assessment of poor neurological prognosis in patients.

A randomized controlled study of subcutaneous Nitroglycerin injection for radial artery catheterization in the snuff box area

Objective To investigate the clinical value of subcutaneous Nitroglycerin injection in radial artery catheterization through the snuff box area. Methods A total of 200 patients with percutaneous coronary angiography in Zhuhai Hospital Guangdong Provincial Hospital of Traditional Chinese Medicine Zhuhai Hospital from July 2021 to February 2023 were selected, they were randomly divided into control group (102 cases) and study group (103 cases) by digital parity method. The control group was injected a mixture of subcutaneously with Lidocaine and saline solution at the puncture site, while the study group was injected a mixture of Lidocaine and Nitroglycerin subcutaneously at the puncture site. The length and short diameter of the artery lumen, the successful proportion of first needle puncture, the number of puncture needles, the successful time of catheterization, the success rate of catheterization, the incidence of vasospasm, the proportion of sheath tube types, the incidence of hematoma, and the incidence of radial artery occlusion 24 h after surgery. Results There were no significant differences in mean arterial pressure difference, heart rate difference between two groups after administration (P>0.05). The incidence of vasospasm in the study group was lower than that in the control group, the success rate of catheterization in the study group was higher than that in the control group, the difference was statistically significant (P<0.05). After local anesthesia, the length and short diameter of the radial artery lumen in the snuff bottle area of the study group were significantly longer than those before local anesthesia, the differences were statistically significant (P<0.05), while the difference was not statistically significant in the control group (P>0.05). After local anesthesia, the length of the radial artery lumen in the snuff bottle area of the study group was longer than that of the control group, the difference was statistically significant (P<0.05). There were no significant differences in the proportion of sheath type, successful proportion of first needle puncture, number of puncture needles, successful catheterization time, intraoperative heparin dosage, and pressure hemostasis time among groups (P>0.05). There was no significant difference in the incidence of postoperative vasospasm, operative hematoma, forearm hematoma, near radial occlusion, far radial occlusion, dissection, and ultrasonic lumen diameter between the two groups (P>0.05). Conclusion Subcutaneous injection of Nitroglycerin before radial artery catheterization through the snuff pot area can dilate the artery, reduce the occurrence of radial artery spasm before catheterization, improve the success rate of catheterization, and have good safety, which is worthy of clinical popularization.

Effect of electroacupuncture preconditioning on the expression of mitop-hagy related proteins in myocardial ischemia-reperfusion injury rats

Objective To investigate the effect of electroacupuncture preconditioning on the expression of autophagy-related proteins in myocardial ischemia-reperfusion injury rats’ mitochondria. Methods Thirty-six male SD rats, aged 8-10 weeks and with body weight 180-220 g were divided into control group, model group, and electroacupuncture pretreatment group by random number table method. The rat myocardial ischemia-reperfusion injury model was developed by an outside-in perfusion system. Rats in control group were only perfused for three hours. Rats in model group and electroacupuncture pretreatment group were perfused for 20 minutes,then ischemia for 40 minutes, and reperfusion for two hours. The electroacupuncture pretreatment group underwent electroacupuncture treatment for three weeks before modeling. The contents of creatine kinase (CK), creatine kinase (CK-MB), and lactate dehydrogenase (LDH) were detected in myocardial tissue; hematoxylin-eosin (HE) staining was stained to visualize the lesions in the groups of cardiomyocytes. Mitochondrial autophagy-related proteins Beclin-1, P62, PINK1, and Parkin exposure were identified by Western blot. Results The contents of CK, CK-MB, and LDH in the model group were higher than those in the control group, with statistical significances (P<0.05); the contents of CK,CK-MB, and LDH in the electroacupuncture pretreatment group were less than those in the model group, with statistical significances (P<0.05). HE staining results indicated that cardiac muscle fibers in the model group were mostly fractured, degenerated, necrotic, and arranged loosely and irregularly.The myocardial tissue structure in the electroacupuncture pretreatment group was basically intact, the intercellular edema was mild, the muscle fiber space was slightly widened, and the necrotic focus was lightened. The levels of mitochondrial autophagy marker proteins Beclin-1, P62, PINK1, and Parkin in model group were higher than those in control group, with statistical significances (P<0.05). The levels of mitochondrial autophagy marker proteins Beclin-1, PINK1, and Parkin in electroacupuncture pretreatment group were higher than those in model group, and the level of P62 was lower than those in model group, with statistical significances (P<0.05). Conclusion Electroacupuncture pretreatment can improve myocardial ischemia/reperfusion injury by regulating the expression of PINK1 and Parkin protein, and activating mitochondrial autophagy.

Influence of epidural analgesia on intrapartum fever and the construction of fever risk prediction model

Objective To research the influence of epidural analgesia on pregnancy outcomes, and to established a risk prediction model of intrapartum fever. Methods A total of 130 pregnant women who voluntarily underwent epidural analgesia delivery in Huaian Maternal and Child Health Hospital, Jiangsu Province (hereinafter referred to as “our hospital”) from April 2018 to April 2022 were selected as the modeling group. They were divided into intrapartum fever group and non-intrapartum fever group according to whether there was intrapartum fever. In addition, 60 pregnant women who underwent epidural analgesia delivery during the same period in our hospital were selected as the validation group. The influencing factors of intrapartum fever were analyzed, and a risk prediction model for predicting intrapartum fever was established, receiver operating characteristic (ROC) curve was used to evaluate the value of the prediction model, and calibration curve and decision curve were established. Results Among 130 pregnant women of epidural analgesia delivery, 44 cases had intrapartum fever. The proportion of oxytocin use, amniotic fluid contamination, and continuous epidural administration in the intrapartum fever group was higher than that in the non-intrapartum fever group, the number of vaginal examination was higher than that the non-intrapartum fever group, and the time from rupture of fetal membrane to the end of delivery was longer than that in the non-intrapartum fever group (P<0.05). The use of oxytocin, the number of vaginal examination, the time from rupture of fetal membranes to the end of delivery, and the method of epidural administration were the influencing factors of intrapartum fever (OR=4.297, 4.660, 3.939, 3.307, P<0.05). Risk prediction model Yprediction= 1.458×the use of oxytocin +1.539× the number of vaginal examination+1.371×the time from rupture of fetal membranes to the end of delivery+1.196×the method of epidural administration. ROC curve showed that the maximum Youden index was 0.501 and the area under the curve was 0.793. The calibration curve showed that the predicted probability was in good agreement with the actual probability. The decision curve showed that the prediction model had good clinical applicability. Conclusion The use of oxytocin, the number of vaginal examinations, the time from rupture of fetal membrane to the end of delivery, and the method of epidural administration are all factors that affect intrapartum fever during epidural analgesia, and the risk prediction model based on this has high prediction efficiency and good clinical applicability, which is conducive to the early diagnosis and treatment of intrapartum fever.

Meta-analysis of methylene tetrahydrofolate reductase gene C677T polymorphism and susceptibility of non-alcoholic fatty liver disease

Objective To investigate the relationship between C677T polymorphism of methylene tetrahydrofolate reductase (MTHFR) gene and susceptibility of non-alcoholic fatty liver disease (NAFLD). Methods PubMed, Cochrane Library, Wanfang Data, CNKI, China Biomedical Literature Database, and VIP were searched by computer to collect literatures on the association between MTHFR gene polymorphism and NAFLD susceptibility. The search time was from the inception to March 2023. In the T vs C, TT vs CC, TT vs CT+CC, TT+CT vs CC, CT vs CC, and TT vs CT gene models, the OR value and 95%CI were used as effect indicators for meta-analysis, and the publication bias and sensitivity analysis were tested by Stata 12.0 software. Results A total of 13 literatures were included, including 5 542 patients with NAFLD and 3 699 controls. Meta-analysis results showed that the risk of NAFLD was increased in five genetic models (T vs C: OR=1.29, 95%CI[1.09-1.52], P=0.003; TT vs CC: OR=1.65, 95%CI[1.23-2.23], P=0.001; TT vs CT+CC: OR=1.48, 95%CI[1.15-1.91], P=0.003; CT+TT vs CC: OR=1.31, 95%CI[1.06-1.62], P=0.013; TT vs CT: OR=1.40, 95%CI[1.09-1.80], P=0.009), heterozygote model CT vs CC was not associated with the risk of NAFLD (OR=1.21, 95%CI[0.98-1.48], P=0.070). The Egger test and funnel plot results showed no publication bias (P>0.05). Meta-regression analysis and sensitivity analysis did not find significant sources of heterogeneity (P>0.05). Conclusion The polymorphism of MTHFR gene C667T is associated with NAFLD, and TT genotype has an increased risk of NAFLD.

Research progress on the mechanism of pulmonary hypertension and myocardial electrical remodeling

Pulmonary arterial hypertension (PAH) is a disease that increases pulmonary arterial pressure and pulmonary vascular resistance leading to right ventricular failure (RVF) or even death of patients. RVF is the most common cause of death in patients with PAH. A large number of studies have shown that the process of RVF in PAH patients involves a series of myocardial remodeling, including cardiac structural remodeling, mechanical remodeling, and myocardial electrical remodeling, and myocardial electrical remodeling is an important cause of patients with malignant arrhythmia and sudden cardiac death. In recent years, there have been many studies on the molecular genetics and cell biology of PAH. This article reviews the upstream pathophysiological mechanism and the biological function and role of myocardial electrical remodeling in the pathogenesis of PAH, so as to provide new targets for the clinical treatment of PAH.

Research progress on the related mechanisms of Apelin-13 in cognitive dysfunction

Apelin is a bioactive peptide and an endogenous ligand for the angiotensin Ⅱ type one receptor-related protein APJ. Its precursor can be cleaved into various active fragments by proteases, with Apelin-13 having the strongest affinity with APJ. Apelin-13 is widely present in the central nervous system and exerts neuroprotective effects by regulating cell apoptosis, autophagy, synaptic plasticity, neuroinflammation, and oxidative stress. Neurons play an important role in the development and maturation of the nervous system, and neuronal damage is closely related to cognitive dysfunction. Therefore, the neuroprotective effect mediated by Apelin-13 has great potential for improving cognitive impairment caused by various reasons. This article reviews the latest research progress of Apelin-13 in cognitive impairment, which is expected to provide new targets and strategies for the treatment of cognitive impairment.

Research progress of moxibustion in the prevention and treatment of epidemic diseases

Epidemic diseases are characterized by rapid spread, rapid onset, and dangerous conditions, which bring heavy burden to the global economy and health care system. For thousands of years, Chinese medicine has formed a complete theoretical system in the prevention and treatment of epidemic diseases. Ancient medical books record a large number of epidemic prevention ideas and treatment measures related to moxibustion, and modern pharmacological research also shows that moxibustion has anti-inflammatory, immune, antibacterial, and antivirus effects. This article is based on the understanding of epidemic diseases in traditional Chinese medicine and combined with modern clinical application research, exploring the feasibility of moxibustion therapy in the prevention and treatment of epidemic diseases from the perspective of “therapy for preventive treatment of disease”, providing evidence and ideas for further prevention and treatment of epidemic diseases in later clinical practice.

Research progress on the immune regulation function and mechanism of astragalus polysaccharides in bronchial asthma

The imbalance of immune cells and the cytokine “storm” it induces are the main driving factors for the occurrence and acute attack of bronchial asthma(“asthma” for short), and the regulation of immune cell imbalance and cytokine“storm” is the fundamental treatment of asthma. Traditional Chinese medicine and its components have immune regulatory functions and can maintain the immune balance in vivo through various ways. Astragalus polysaccharides is the active ingredient of traditional Chinese medicine Huangqi, which has anti-inflammatory and immunomodulatory functions. Based on the interaction of astragalus polysaccharides receptors, this paper reviewed the molecular mechanisms of astragalus polysaccharides regulating MAPK and NF-κB signaling pathways, and comprehensively summarized its immunomodulatory effects and mechanisms in asthma, providing a theoretical basis for the clinical application of astragalus polysaccharides in asthma prevention and treatment, as well as a molecular basis for modern research on traditional Chinese medicine Huangqi.

Application progress of antidepressants in inflammatory bowel disease

Inflammatory bowel disease (IBD) is a chronic, recurrent, non-specific intestinal inflammatory disease. Its incidence is increasing year by year and the etiology is unknown. At present, there is no cure. The disease has characteristics of repeated attacks and prolonged, which brings great psychological burden to patients, and the risk of depression and anxiety is greatly increased. Research in recent years has shown that antidepressants can change the course of IBD patients and improve the prognosis. The mechanism of action may be to achieve anti-inflammatory and improve structure through brain-intestinal axis. This article reviews the relationship between antidepressants and IBD in order to promote the study of antidepressants in IBD.

With the incidence rate of urological diseases increasing year by year, how to effectively treat such diseases has become an important topic of clinical research and drug research. Traditional western medicine represented by antibiotics, although fast-acting, but prone to a variety of side effects. Yinhua Miyanling has natural ingredients and modern scientific research has proven its good antibacterial, diuretic, and anti-inflammatory effects, showing excellent efficacy and safety in the treatment of urinary system diseases, and it is widely used. This article will elaborate on the mechanism of action of Yinhua Miyanling from the perspective of basic research, and summarize the effectiveness and advantages of Yinhua Miyanling in treating urinary system diseases from the perspective of clinical research. This article analyzes the current problems and points out future research directions in order to provide more effective and safe drug choices for clinical treatment.

Research progress in etiology and physiotherapy of cervicogenic headache

Cervicogenic headache(CEH) is a secondary headache that arises from the structure of the neck. The pathogenesis of the disease is controversial, the diagnosis lacks objective diagnostic criteria such as imaging, the misdiagnosis rate is high, and the choice of treatment is subjective differences. Studies have found that CEH is mainly caused by structural abnormalities such as physiological curvature, joint structure, neck muscle group, fascial ligament, and dura. Physiotherapy is the most widely used clinical practice in CEH, physiotherapy (manual therapy, exercise therapy, acupuncture, etc.) can improve the symptoms, cervical motion, quality of life, and inflammatory indicators of CEH patients through multi-target effects. This article reviews the research progress in etiology of CEH and physiotherapy, in order to provide reference for the future study of CEH.

Research progress on the effect and therapeutic application of PTEN in lung cancer

PTEN is an important tumor suppressor gene of lung cancer, which inhibits the activation of AKT and mTOR by inhibiting PIP3 production. On the one hand, it directly inhibits tumor cells. On the other hand, it weakens the supporting effect of tumor stroma on tumor cells, and inhibits the occurrence, growth, and metastasis of lung cancer. Decreased expression of PTEN gene leads to the development of drug resistance in lung cancer treatment and reflects the prognosis of patients. However, due to the existence of epigenetic and post-translational regulation, PTEN protein deletion is significantly higher than the incidence of PTEN gene mutation. Therefore, compared with PTEN gene targeting therapy drugs, improving the expression of PTEN protein in lung cancer may be a more promising approach to PTEN gene therapy for lung cancer and inhibiting drug resistance.

Meta-analysis of the application effect of curriculum ideology and politics in the teaching of medical specialty

Objective To evaluate the application effect of curriculum ideology and politics in medicine specialty by meta-analysis method. Methods By searching Wanfang Data, CNKI, and VIP databases, a combined search was conducted based on keywords such as curriculum ideology and politics, medicine, education, and academic performance, and the search time was set to be from January 1, 2013 to December 31, 2022. After the inclusion, and exclusion criteria were set, the literature was screened, and information such as academic performance and satisfaction in the literature was extracted as observation indicators. A meta-analysis of the data was performed using R 4.2.2. Results A total of 25 literatures were included in this meta-analysis, with a total sample size of 6 177 participants, including 754 participants from the traditional teaching mode group and 5 423 participants from the curriculum ideological and political teaching group. The results of meta-analysis showed that the test scores of students in the ideological and political teaching group were higher than those in the traditional teaching mode group, and the differences were statistically significant (MD=4.77, 95%CI: 3.61-5.93, Z=8.05, P< 0.001). The students have a high degree of satisfaction (83.40%, 95%CI: 82.10%-84.67%) with the teaching methods of ideological and political courses, and thought that they could improve the mastery of professional knowledge, improve learning interest, cultivate thinking ability and innovation ability, the proportions were 85.34%, 89.15%, 90.29%, and 72.38%, respectively. Conclusion The teaching mode based on curriculum ideology and politics can improve the academic performance of medical students, and has a high degree of satisfaction and identity, which can be popularized in the teaching of medical majors.

Ideological and political design and implementation of standardized admission education course for resident doctors

Objective To carry out the course of ideological and political education in the series of hospitalization education courses for residents, evaluate the teaching effect, and explore the implementation path of ideological and political education in standardized training for residents (“residential training” for short). Methods In accordance with the guidelines, admission education for newly admitted residents will be carried out in August 2022. Assessment and teaching evaluation were carried out after admission education. Results The passing rate of 89 resident doctors was 100%. A total of 85 residents completed the teaching evaluation. The average score of 20 items was above 4.80 points; 85.88% of residents rated the course as “extremely high” for “improving understanding of medical humanities” higher than the other nine entries. Of the five courses that impressed residents most, four covered ideological and political elements. The results showed that admission education strengthened the concept of respecting life, paying attention to patient communication and humanistic care. Conclusion Admission education can realize the education and guidance of medical spirit simultaneously with the training of professional knowledge and skill operation for residents. Admission education should be regarded as the first lesson of ideological and political education in residential training, which is worthy of further exploration and optimization in practice.

Examination of the problems and innovative paths of traditional health sports teaching in schools from the perspective of “healthy China”

The alignment of traditional physical health education with the “healthy China” strategy has far-reaching significance, which is conducive to improving the physical quality of the nation and ensuring the smooth implementation of the national health strategy. This paper uses literatures and logical analysis to analyze the inner connection between traditional physical education and health care courses and the “healthy China” strategy, and focuses on the problems and reform directions of traditional health care physical education in the context of “healthy China”, and puts forward rational suggestions. The study finds that traditional health care physical education in the context of “healthy China” is in line with the national strategic development needs, helps to reduce the social medical burden, and can effectively improve the health level of individuals, and other important roles. However, there are also problems of lagging teaching philosophy, teaching system to be improved and updated, and unbalanced knowledge structure of teachers. Therefore, it is necessary to update the teaching objectives and contents of “moral-skill-competence”, construct the teaching mode of “triple classroom”, improve the teaching evaluation system, and improve the comprehensive ability of teachers. The four aspects of innovation and development should be carried out.

Clinical effect of supramolecular salicylic acid on acne rosacea and its influence on inflammatory factors

Objective To observe the clinical effect of supramolecular salicylic acid on acne rosacea and its influence on inflammatory factors. Methods Eighty-six patients with acne rosacea treated in the Second People’s Hospital of Huaian, Jiangsu Province were selected as the study objects, and were divided into microneedle group (43 cases) and microneedle + salicylic acid group (43 cases) according to random number table method. Both groups were treated with antibiotics, the microneedles group was treated with gold microneedles on the basis of antibiotics, and the microneedles + salicylic acid group was treated with supramolecular salicylic acid on the basis of microneedles group. After continuous treatment for eight weeks, the differences of inflammatory factors, skin physiological function, symptom score, and quality of life between the two groups were compared, and the efficacy and safety of the two groups were analyzed. Results In the microneedle + salicylic acid group, six cases fell off during treatment, and 37 cases were eventually included. After treatment, trans epidermal water loss, skin erythematous index, skin oil, interleukin (IL)-4, IL-6, IL-37, and tumor necrosis fact-α (TNF-α) in two groups were lower than those before treatment, and those of microneedles + salicylic acid group were lower than those of microneedles group, the differences were statistically significant (P<0.05). After treatment, cuticle water content and acne-specific quality of life questionnaire scores in two groups were higher than those before treatment, and those of microneedles + salicylic acid group were higher than those of microneedles group, the differences were statistically significant (P<0.05). After treatment, the scores of flushing, erythema, papule, pustules, and telangiectasia in two groups were lower than those before treatment, and those of microneedle + salicylic acid group were lower than those of microneedle group, the differences were statistically significant (P<0.05). The therapeutic effect of microneedle + salicylic acid group was better than that of microneedle group, and the difference was statistically significant (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05). Conclusion Supramolecular salicylic acid and gold microneedles in the treatment of rosacea can improve skin physiological function, inhibit inflammatory response, alleviate symptoms, and improve quality of life.

Predictive efficacy of serum macrophage inflammatory protein-3α com-bined with receptor interacting protein kinase 3 levels for the short-term prognosis of patients with hepatitis B virus-related acute-on-chronic liver failure

Objective To investigate the predictive efficacy of serum macrophage inflammatory protein-3α (MIP-3α) combined with receptor interacting protein kinase 3 (RIPK3) on the short-term prognosis of patients with hepatitis B virus-related acute-on-chronic liver failure (HBV-ACLF). Methods A total of 246 patients with HBV-ACLF admitted to the Second Hospital of Nanjing from February 2021 to July 2022 were selected. The serum levels of MIP-3α and RIPK3 were detected. According to the survival situation within 30 days, the patients were divided into the death group (73 cases) and the survival group (173 cases). The clinical data of the two groups were compared, and the factors affecting the short-term prognosis of patients with HBV-ACLF and the predictive value of MIP-3α and RIPK3 for the prognosis of patients with HBV-ACLF were analyzed. Results The serum albumin level in the death group was lower than that in the survival group, the international normalized ratio, model for end-stage liver disease (MELD) score, the proportion of complicated encephalopathy, and the serum total bilirubin, MIP-3α, and RIPK3 levels were higher than those in the survival group (P<0.05). MELD score (OR=3.347, 95%CI: 1.844-6.073), and serum MIP-3α (OR=2.079, 95%CI: 1.307-3.309), RIPK3 (OR=2.004, 95%CI: 1.312-3.060) were influential factors for short-term prognosis of HBV-ACLF patients (P<0.05). MELD score combined with serum MIP-3α and RIPK3 predicted the prognosis of HBV-ACLF patients with higher area under the receiver operator charcateristic curve than the area under the curve predicted by the three alone (P<0.05). Conclusion Serum MIP-3α and RIPK3 levels are elevated in HBV-ACLF patients, and are associated with short-term death. Combined analysis of MELD score and serum MIP-3α, RIPK3 helped to indicate the prognosis of patients.

Clinical evaluation of serum gastric function detection for warning of NSAIDS-related gastric hemorrhage

Objective To investigate the predictive value of serum gastric function indexes in patients with non-steroidal anti-inflammatory drugs (NSAIDs) related gastric hemorrhage. Methods A total of 191 patients with chronic diseases who received NSAIDs (Aspirin) in the Second People’s Hospital of Jingdezhen City, Jiangxi Province from May 2020 to May 2022 were selected as the study objects. According to the occurrence of gastric hemorrhage during three months of treatment, they were divided into bleeding group and no bleeding group. The levels of serum gastric function indexes before and after treatment were compared between the two groups. The influencing factors of gastric hemorrhage and the prediction effect of serum gastric function index on gastric hemorrhage were analyzed. Results The incidence of gastric hemorrhage within three months after NSAIDs treatment was 9.95%. There were statistically significant differences in age, history of alcoholism, history of digestive tract and, use of gastric mucosal protective agents between the two groups (P<0.05). There were statistically significant differences in serum gastrin-17 (G-17), pepsinogen (PG) Ⅰ, PGⅡ, and PGⅠ/PGⅡ between the two groups (P<0.05). After two months of treatment, the G-17 level in the bleeding group was lower than that before and after one month of treatment, and lower than that in the non-bleeding group. The levels of PGⅠ, PGⅡ, and PGⅠ/PGⅡ were higher than those before treatment and after one month of treatment, and higher than those in non-bleeding group, the differences were statistically significant (P< 0.05). Logistic regression analysis showed that age, history of alcoholism, history of digestive tract, gastric mucosal protective agents, serum G-7, PG-Ⅰ, PG-Ⅱ, and PG-Ⅰ/PG-Ⅱ levels after two months of treatment were all influential factors in the occurrence of gastric hemorrhage (P<0.05). After two months of treatment, the AUC of serum G-17, PG-Ⅰ, PG-Ⅱ, and PG-Ⅰ/PG-Ⅱ combined predicted gastric hemorrhage was 0.879, which was higher than that predicted by any single index, and the differences were statistically significant (P<0.05). Conclusion The different expression of serum gastric function index in patients with NSAIDs related gastric hemorrhage, combined detection of serum gastric function index has a certain predictive value for gastric hemorrhage.

Application effect of thromboela-stogram in blood transfusion guidance for patients with traumatic massive hemorrhage and severe coagulation disorder

Objective To investigate the application of thromboela-stogram to guide blood transfusion in patients with traumatic massive hemorrhage and severe coagulation disorder. Methods A total of 107 patients with traumatic massive hemorrhage with severe coagulation disorder were selected from Lianyungang Second People’s Hospital of Lianyungang City, Jiangsu Province from December 2018 to December 2021, and were divided into routine group (54 cases with routine coagulation function detection) and study group (53 cases with thromboela-stogram detection) by random number table method. Activated partial thrombin time (APTT), prothrombin time (PT), thrombin time (TT), and fibrinogen (FIB) were measured before and after blood transfusion. The amounts of platelets, plasma, red suspension, and cryoprecipitate were counted during blood transfusion. The duration of hospitalization and ICU treatment and the incidence of complications during hospitalization were analyzed. Results After transfusion, APTT, PT, and TT in two groups were shorter than those before transfusion, while those in the study group were lower than those in the routine group; FIB was higher than that before transfusion, while that in the study group was higher than that in the routine group, and the differences were statistically significant (P<0.05). The dosage of platelet, plasma, red suspension, and cryoprecipitate in the study group were lower those that in the routine group, and the differences were statistically significant (P<0.05). There was no significant difference in hospital stay between the two groups (P>0.05). The duration of ICU stay in the study group was shorter than that in the routine group, and the difference was statistically significant (P<0.05). There was no significant difference in the total incidence of complications between the two groups (P>0.05). Conclusion Thromboelastogram examination has a certain guiding value in guiding blood transfusion in patients with traumatic hemorrhage and severe coagulation disorder.

Application of ultrasound-assisted horizontal minimally invasive breast gyrotomy in excision of superficial breast masses

Objective To investigate the application effect of ultrasonic-assisted horizontal minimally invasive breast gyrotomy in excision of superficial breast masses. Methods A total of 86 patients with superficial breast masses admitted to Shengzhou Hospital of Traditional Chinese Medicine from January 2019 to March 2022 were selected and they were divided into study group and control group by random number table method, with 43 cases in each group. The control group received traditional vertical minimally invasive breast gyrotomy, while the study group received ultrasound-assisted horizontal minimally invasive breast gyrotomy. The perioperative related indicators, postoperative pain, postoperative complications, cosmetic effects, and prognosis were compared between the two groups. Results There was no significant difference in incision length and healing time between the two groups (P>0.05). The operation time of the study group was shorter than that of the control group, the intraoperative blood loss in the study group was lower than that in the control group (P<0.05). The visual analogue scale (VAS) scores of both groups were lower at 24 h after surgery than that at 6 h after surgery (P<0.05), and the VAS score of the study group was lower than that of the control group at 6 and 24 h after surgery (P<0.05). The total incidence of complication of the study group was lower than that of the control group (P<0.05). The vancouver scar scale score of the study group was lower than that of the control group at six months after surgery (P<0.05). Six months after surgery, there was no difference in the residual rate of tumor between the two groups (P>0.05). Conclusion Ultrasound-assisted horizontal minimally invasive breast gyrotomy is effective in resection of superficial breast tumors. It can shorten the operation time, reduce the surgical trauma and postoperative pain, promote the recovery of patients, and improve the cosmetic effect. The prognosis is good, and it is safe and reliable.

Effect of ultrasound-guided quadratus lumbois block on early outcome after renal transplantation

Objective To investigate the effect of ultrasound-guided quadratus lumbois block on early outcome of renal transplantation. Methods Sixty patients with end-stage renal disease who underwent kidney transplantation at the 924th Hospital of the Joint Logistics Support Force of the People’s Liberation Army of China from January 2021 to January 2023 were selected, according to the random number table method, they were divided into quadratus lumbois block group (group Q) and transversal muscle plane block group (group T), with 30 patients in each group. After successful induction of anesthesia, group Q was subjected to ultrasound-guided quadratus lumbois, group T was subjected to ultrasound-guided transversal muscle plane block. Heart rate (HR) and mean arterial pressure (MAP) were compared immediately before skin incision and five minutes after skin incision; the intraoperative doses of Sufentanil and Remifentanil were compared between two groups; the postoperative recovery indexes of two groups were compared; visual analog scale (VAS) score was compared between two groups extubation for 15 min, 2 h after surgery, 12 h after surgery, 24 h after surgery, and 48 h after surgery; complications during postoperative analgesia, effective compression times, and remedial analgesia were compared between two groups; the changes of stress inflammatory response before anesthesia and 48 h after surgery were compared between two groups. Results There were no statistically significant difference in HR and MAP intra-group and inter-group comparison between two groups immediately before skin incision and five minutes after skin incision (P>0.05). The intraoperative doses of Sufentanil and Remifentanil of group Q were lower than those of group T, and the differences were statistically significant (P<0.05). The postoperative resuscitation and extubation time, first time out of bed, and hospitalization time of group Q were shorter than those of group T, and the differences were statistically significant (P<0.05). The overall analysis showed that there were statistically significant differences in the inter-group, time, and interaction of VAS between two groups (P<0.05). Intra-group comparison: VAS scores at 2, 12, 24 h, and 48 h after surgery in both groups were higher than those at extubation of 15 min; VAS scores at 12, 24 h, and 48 h after surgery in both groups were higher than those at 2 h after surgery; VAS scores at 24 h and 48 h after surgery in both groups were higher than those at 12 h after surgery; VAS scores at 48 h after surgery in both groups were lower than those at 24 h after surgery, and the differences were statistically significant (P<0.05). Comparison between groups: VAS scores of group Q at 2, 12, 24 h, and 48 h after surgery were lower than those of group T, and the differences were statistically significant (P<0.05). There were no significant differences in the total incidence of complications and the rate of remedial analgesia between two groups (P>0.05). The effective compression times of group Q was less than that of group T, and the difference was statistically significant (P<0.05). The levels of cortisol, C-reactive protein, interleukin-6, and tumor necrosis factor-α at 48 h after surgery in two groups were higher than those before anesthesia, and those of group Q were lower than those of group T, and the differences were statistically significant (P<0.05). Conclusion Ultrasound-guided quadratus lumbois block can accelerate the early recovery after kidney transplantation, the postoperative analgesia is significant, and the effect of stress and inflammatory response is small.

Effects of perioperative period thermal insulation intervention on stress response and complications in patients undergoing thoracoscopic lung surgery under general anesthesia

Objective To investigate the effects of perioperative period thermal insulation intervention on stress response and complications in patients undergoing thoracoscopic lung surgery under general anesthesia. Methods A total of 110 patients with thoracoscopic lung surgery treated in Huaian First People’s Hospital, Jiangsu Province from October 2019 to November 2022 were selected as the study objects, and they were divided into control group (55 cases) and observation group (55 cases) by random number table method. The control group received routine intervention in perioperative period, and the observation group received thermal insulation intervention in perioperative period on the basis of control group. Both groups were observed until the patients left the anesthesia recovery room. The body temperature between two groups was compared before anesthesia (T0), 60 min after the operation (T1), at the end of operation (T2), immediately after entering the anesthesia recovery room (T3), 15 min after entering the anesthesia recovery room (T4), and at the exit of the anesthesia recovery room (T5); the levels of norepinephrine (NE), epinephrine (E), and cortisol (Cor) at T0, T2, and T4 were compared between two groups; the occurrence of complications in two groups was observed. Results Overall comparison: there were statistically significant differences in body temperature time, inter-group, and interaction between two groups (P<0.05). Intra-group comparison: compared with T0, the body temperature of control group decreased at T1; compared with T1, the body temperature was increased at T2-T5, and the differences were statistically significant (P<0.05). Compared with T0, the body temperature of observation group was increased at T2-T5, and the differences were statistically significant (P<0.05). Comparison between groups: the body temperature of observation group at T1-T5 was higher than that of control group, and the difference was statistically significant (P<0.05). Overall comparison: there were statistically significant differences in the levels of NE, E, and Cor time, inter-group, and interaction between two groups (P<0.05). Intra-group comparison: the pairwise comparison of the levels of NE, E, and Cor in two groups at different time points had statistical significance (P<0.05). Comparison between groups: the levels of NE, E and Cor at T2, T4 in observation group were lower than those in control group, and the differences were statistically significant (P<0.05). The total incidence of complications in observation group was lower than that in control group, and the difference was statistically significant (P<0.05). Conclusion Perioperative thermal insulation intervention could help maintain the body temperature of patients undergoing thoracoscopic lung surgery, reduce the stress response, postoperative complications of patients, and had a good intervention effect.

The application of auricular point pressing pills, chirismus with Chinese herbal ointment, acupoint application, shibaduanjin, and other traditional Chinese medicine characteristic external treatment can effectively reduce the clinical symptoms of patients and improve the clinical effect. Professor Zhao Jinxi, based on the pathogenesis theory of “micro category of abdominal mass accumulation diseases” of diabetic nephropathy and the understanding of “category of abdominal mass accumulation diseases leading to diseases, void and reality are mutually causal”, uses auri- cular point pressing pills to focus on internal and external treatment, chirismus with Chinese herbal ointment to focus on supplementation of qi to eliminate syndrome, acupoint application to focus on syndrome differentiation and treatment, and practices shibaduanjin to advocate the overall adjustment of the balance of yin and yang of zang fu organs. Adenomyosis is common in women of childbearing age and is invasive, hormone-dependent, with a variety of symptoms and progressive exacerbation and a long course of disease. According to traditional Chinese medicine, it is the stagnation of blood away from the meridian, and treatments are based on activating blood circulation to dissipate blood stasis. With years of clinical experience, Professor Deng Gaopi has found that the collaterals disease theory may better reflect the location, pathogenesis, and course of adenomyosis than the theory of blood stasis. To treat this disease from collaterals disease, on the basis of routine of activating blood circulation to dissipate blood stasis, more emphasis is put on the cause of collaterals disease and the use of collaterals entering drugs. The clinical method is to removing obstruction in collaterals as the method, and the selection of pungent removing obstruction in collaterals drugs, insect drugs, rattan drugs, inducing diuresis and expelling pus removing obstruction in collaterals drugs, tonifying deficiency and nourishing collaterals drugs. At the same time, combined with the physiological characteristics of patients, the main demands of individual treatment, in order to protect the venation, ease the venation, so that the blood does not leave the meridian, the disease blocks slow down.

Clinical effect of acupuncture trigger point treatment for acute ankle sprain under the guidance of traditional Chinese medicine meridian theory

Objective To study the clinical efficacy of acupuncture trigger points in the treatment of acute ankle sprain based on the guidance of traditional Chinese medicine meridian theory. Methods Fifty patients with unilateral acute ankle sprain who visited Yangguang 100 Community Health Service Center from January 2021 to March 2022 were selected and they were divided into control group and treatment group by random number table method, with 25 cases in each group. The control group were treated with ankle triple needle therapy, while the treatment group were treated with acupuncture trigger point therapy. Both groups were treated once every other day, a total of 5 times, with a period of 10 days. The clinical efficacy, visual analogue scale (VAS) score, ankle-hind foot function score, and ankle swelling score were compared between the two groups. Results The clinical effect of the treatment group was better than that of the control group (P<0.05). After treatment, the ankle VAS and swelling scores of both groups were lower than before treatment, and the ankle-hind foot function scores were higher than before treatment, and the ankle pain VAS scores and swelling scores of the treatment group were lower than those of the control group, and the ankle-hind foot function score was higher than that of the control group (P<0.05). Conclusion The use of acupuncture trigger points has a significant therapeutic effect on acute ankle sprain, which can effectively eliminate tissue swelling and improve ankle joint mobility.

Zhengzhi Zhunsheng is a medical book written by Wang Kentang, a physician of the Ming dynasty, which revolutionizes the diagnosis and treatment of breast diseases based on the work of his predecessors, the main etiology and pathogenesis of breast diseases are anxiety and depression, liver depression and qi stagnation, impaired liver and spleen function, accumulation of phlegm and poison, and deficiency of qi and blood, etc. In rare cases of male breast cancer, it is attributed to liver and kidney damage caused by anger and excessive libido. Wang Kentang emphasizes the importance of dialectical treatment, where treatment methods are flexibly applied based on clinical manifestations. He also highlights the significance of emotion therapy, distinguishs the difference in pathogenesis while regulating emotion, emphasizes the importance of examining the actual condition of qi and blood. He propose that milky rock should be detected, diagnosed and treated early, which coincides with the understanding of breast cancer in modern medicine. The analysis of Wang Kentang’s clinical experience in the treatment of milky rock is of enlightening and guiding significance for the syndrome differentiation and treatment of breast cancer.

Discussion on the experience of Wang Yunming in treating diminshed ovarian reserve infertility

This paper explores the etiology and pathology and the summary of treatment of diminshed ovarian reserve in Wang Yunming. Diminshed ovary reserve is one of common clinical pathogeny in infertility, the pathogenesis is complex, and chronic disease is difficult to heal. Wang Yunming believes that the pathogenesis of diminshed ovarian reserve is based on the deficiency of the kidney, the stagnation of qi, which is deficiency in origin and excess in superficislity; disease located in the uterus, kidney, and heart, involving the liver, spleen, and lung, which is connected with governor channel, conception channel, and thoroughfare channel. In terms of treatment, it agrees with the core treatment principle of “nourishing kidney and activating blood and unobstructed vessels”. It advocates tonifying kidney and regulating thoroughfare channel, warming ovary, and nourishing follicle to take care of its root; promoting blood circulation and removing blood stasis, eliminating phlegm and dampness to treat its symptoms, which has taking into account the overall, staging treatment, leading things by trend, coordinating multiple organs, and curing through both internal and external. In medication, Wang Yunming is adept in using sentient flesh and blood products and insect drugs, tonifying Yin and Yang and applying both general and tonic, it provides a new idea and method for treating infertility with decreased diminshed ovarian reserve infertility.

Professor Zhang Xiaojun’s experience in treating plasma cell mastitis during pregnancy by external application of traditional Chinese medicine

Plasma cell mastitis is a chronic inflammatory disease that occurs in women of childbearing age during the non-lactation period, it has the characteristics of sudden onset, diverse condition, and prolonged course. It can also occur during pregnancy. At present, there are few relevant reports, considering the influence of oral drugs and the limited effectiveness of antibiotics, the selection and effectiveness of its treatment are difficult nowadays. For the treatment of plasma cell mastitis during pregnancy, Professor Zhang Xiaojun has rich clinical experience. Using simple external application of traditional Chinese medicine, he created Wentong Xiaoxing Prescription, which directly acts on breast lesions, is simple, safe, and effective. Clinical treatment with local signs and the evolution of the course of disease as the starting point, focusing on the nature of the disease of “root causes feeble but symptoms critical”, specializing in the pathological factors of “phlegm coagulation, blood stasis”, staging syndrome differentiation, accurate treatment, the concept of “warming yang, and ooze pathogenic factor” throughout the treatment. The prescription mainly uses the products of warming yang to dissipate cold, freeing the network vessels, and combines with the herbs of disspating heat, dissolving nodes, thereby helping yang to detoxify, supporting vital qi to get rid of evil, and thus the sore can be healed and the mammary duct can be dredged. He belives that the location of familial adenomatous polyposis is in the intestinum crassum, closely related to the liver, spleen, and kidney. Excess in superficiality is mainly due to the accumulation of phlegm and blood stasis, as well as the heat in the colon and organs. In terms of treatment principles, he mainly focuses on tonifying the kidney and filling the essence, strengthening the spleen and regulating the liver, resolving phlegm and blood stasis, and clearing heat and dispersing nodules. He emphasizes the importance of supporting the right and eliminating evil, and treating both symptoms and root causes simultaneously, and combining disease and syndrome based on the patient’s condition, adding or subtracting with the syndrome.

Diagnostic value of functional magnetic resonance imaging combined with cortisol, homocysteine in type Ⅱ schizophrenia

Objective To analyze the diagnostic value of functional magnetic resonance imaging (fMRI) combined with cortisol and homocysteine (Hcy) in type Ⅱ schizophrenia. Methods Sixty patients with schizophrenia diagnosed at Shaoxing Seventh People’s Hospital (“our hospital” for short) from April 2021 to January 2022 were selected as the research group. Among them, 37 cases of type Ⅰ schizophrenia were the type I group, and 23 cases of type Ⅱ schizophrenia patients were the type Ⅱ group. Another 60 healthy individuals who underwent normal physical examinations in our hospital during the same period were selected as the control group. fMRI was performed and cortisol and Hcy were measured. The diagnostic value of fMRI combined with cortisol and Hcy in type Ⅱ schizophrenia were analyzed. Results The transverse diameter, left lateral crack width, and right lateral crack width of ventricle Ⅲ in type Ⅱ group were higher than those in control group and type Ⅰ group, and the thickness of corpus callosum was lower than those in control group and type Ⅰ group, and the differences were highly statistically significant (P<0.05). There was no significant difference in fMRI indexes between group Ⅰ and control group (P>0.05). The cortisol level of type Ⅱ group was significantly lower than that of control group and type Ⅰ group, and the differences were highly statistically significant (P<0.01). There was no significant difference in cortisol level between group Ⅰ and control group (P>0.05). The level of Hcy in type Ⅰ and type Ⅱ groups was higher than that in control group, and the level of Hcy in type Ⅱ group was significantly higher than that in type Ⅰ group, the differences were highly statistically significant (P<0.01). The area under the curve value of fMRI combined with cortisol and Hcy in the diagnosis of type Ⅱ schizophrenia was higher than that of other tests alone. Conclusion fMRI examination results, cortisol, and Hcy abnormalities in patients with type Ⅱ schizophrenia, and the combination of fMRI and cortisol and Hcy detection can effectively improve the diagnostic value of negative schizophrenia.

Value of ultrasound-guided modified needle-knife in the treatment of adhesive capsultis of shoulder with bursal effusion

Objective To explore the value of ultrasound-guided modified needle-knife in the treatment of adhesive capsultis of shoulder (ACS) with bursal effusion. Methods A total of 112 patients with ACS were selected from the Second Provincial People’s Hospital, Gansu Province from February 2020 to February 2022. They were divided into the needle group and the control groups by random number table method, with 56 cases in each group. The needle group was treated with ultrasound-guided modified needle-knife therapy, while the control group was treated with traditional needle-knife therapy. Both group were treated once a week for two weeks. The clinical effect of the two groups was compared; visual analogue scale (VAS) scores, disabilities of the arm, shoulde, and hand (DASH) scores, and shear wave elastic velocity (SWV) were compared between the two groups before treatment (T1), treatment for two weeks (T2), two weeks after treatment (T3), four weeks after treatment (T4); follow-up for one year, the recurrence of the two groups was compared. Results The total effective rate of the needle group was higher than that of the control group (P<0.05). Overall comparison: VAS score, DASH score, SWV among groups, time, and interaction were statistically significant between the two groups (P<0.05). Intra-group comparison: at T2-T4, VAS score, DASH score, and SWV of the two groups were lower than those at T1 (P<0.05). Comparison between groups: at T2-T4, VAS score, DASH score, and SWV in the needle group were lower than those in the control group (P<0.05). The recurrence rate of the needle group was lower than that of the control group (P<0.05). Conclusion Ultrasound-guided modified-knife has better efficacy and lower recurrence rate in the treatment of ACS, which can be applied in clinic.

Value of ultrasonic elastography, CEA, and CA125 in differential diagnosis of malignant breast nodules with rear echo attenuation

Objective To investigate the value of ultrasonic elastography, carcinoembryonic antigen (CEA), and carbohydrate antigen 125 (CA125) in differential diagnosis of malignant breast nodules with rear echo attenuation. Methods A total of 76 patients with rear echo attenuation breast nodules admitted to Suzhou Hospital of Integrated Chinese and Western Medicine, Jiangsu Province from May 2016 to February 2022 were selected. All patients underwent ultrasonic elastography. The ultrasonic elastography characteristics (strain ratio of fat/strain ratio of nodules [B/A], minimum elastic modulus [Emin], average elastic modulus [Emean], and maximum elastic modulus [Emax]) of benign and malignant breast nodules were compared; the levels of CEA and CA125 of benign and malignant breast nodules were compared; receiver operating characteristic (ROC) curve was used to analyze the diagnostic value of ultrasound elastography, CEA, and CA125 in breast nodules with rear echo attenuation. Results Among 76 patients with breast nodules, 29 cases were malignant and 47 cases were benign. The diameter of malignant breast nodules was larger than that of benign nodules (P<0.05). B/A, Emean, and Emax of breast malignant nodules were higher than those of benign nodules (P<0.05); there was no significant difference in Emin between benign and malignant breast nodules (P>0.05). The levels of CEA and CA125 of malignant breast nodules were higher than that of benign nodules (P<0.05). The area under the curve of malignant breast nodules detected by B/A, Emean, Emax, CEA, and CA125 was higher than that detected by each index alone. Conclusion Ultrasound elastography (B/A, Emean, and Emax), CEA, and CA125 are important in the differential diagnosis of malignant breast nodules with rear echo attenuation.

Application effect of enhanced recovery after surgery nursing on postoperative delirium and cognitive dysfunction in elderly patients with biliary tract

Objective To explore application effect of enhanced recovery after surgery (ERAS) nursing on postoperative delirium and cognitive dysfunction in elderly patients with biliary tract. Methods A total of sixty patients with biliary tract surgery admitted to the Department of Hepatobiliary Surgery of Wenzhou Central Hospital, Zhejiang Province from July 2022 to December 2022 were included, they were divided into control group and ERAS group by random number table method, with 30 cases in each group. The control group received general surgery basic nursing, ERAS group received ERAS mode nursing program on the basis of the control group. The incidence of postoperative delirium, cognitive function score, and postoperative adverse reactions were observed. Results After surgery, postoperative delirium incidence in ERAS group was lower than that in control group, and delirium duration in ERAS group was shorter than that in control group, and the differences were statistically significant (P<0.05). The overall analysis showed that there were statistical significances in comparison between groups, comparison at time points and interaction of mini-mental state examination (MMSE) between the two groups (P<0.05). Intra-group comparison showed that when the MMSE scores of the two groups were lower at 1, 3, and 7 days after surgery, the MMSE scores at 3 and 7 days after surgery were higher than those at 1 day after surgery, the MMSE scores at 1 and 3 months after surgery were higher than those at 3 days after surgery, and the MMSE scores at 1 and 3 months after surgery were higher than those at 7 days after surgery, the differences were statistically significant (P<0.05). MMSE scores at 1, 3 and 7 days after operation in ERAS group were higher than those in control group, the differences were statistically significant (P<0.05). The incidence of nausea and vomiting, urinary retention, sleep disorders and hypotension in ERAS group was lower than that in control group, and the duration of postoperative hospitalization was shorter, the differences were statistically significant (P<0.05). Conclusion Compared with the conventional nursing scheme, the nursing ERAS can effectively reduce the occurrence of postoperative delirium, shorten the duration of delirium, reduce postoperative cognitive dysfunction and promote the recovery of cognitive function in elderly patients with biliary tract surgery, and is worthy of clinical promotion and application.

Effect of load breathing training combined with high-purity negative oxygen ions on lung function and rehabilitation in patients with chronic obstructive pulmonary disease

Objective To observe the effect of load breathing training combined with high-purity negative oxygen ions on lung function and rehabilitation in patients with chronic obstructive pulmonary disease (COPD). Methods A total of 150 COPD patients were admitted to the Second People’s Hospital of Wuxi, Jiangsu Province from January 2021 to November 2022. According to the method of random number table, they were divided into control group and study group, with 75 cases in each group. The control group was given six weeks of loaded respiratory training, while the research group received an additional two weeks of high-purity negative oxygen ion therapy based on the treatment of the control group. The lung function, blood gas indices, respiratory muscle contractility, and rehabilitation indicators of both groups were observed and compared. Results After intervention, the forced vital capacity, maximum ventilation capacity, forced expiratory volume in one second, partial pressure of oxygen, oxygen saturation, maximum suction pressure, and other indicators of the two groups were higher than before intervention, and the study group were higher than the control group ( P<0.05). The 6 min walking distance was longer than that before intervention, and that of the study group was longer than that of the control group (P<0.05). The partial pressure of carbon dioxide, maximum expiratory pressure, chronic obstructive pulmonary disease assessment test, and modified UK MRC respiratory difficulty index rating scale scores were lower than those before intervention, and those of the study group were lower than those of the control group (P<0.05). Conclusion High purity negative oxygen ion combined with load breathing training for COPD patients can improve the pulmonary function, improve the strength of respiratory muscles, and promote the rapid recovery of patients.

Objective To explore the application effect of graded pain nursing in patients with acute pancreatitis treated with raw rhubarb enema. Methods A total of 92 patients with acute pancreatitis admitted to the Department of Spleen and Stomach Diseases, Rugao Hospital of Traditional Chinese Medicine, Jiangsu Province from January 2019 to January 2022 were selected as the study objects, according to random number table method, they were divided into reference group and intervention group, with 46 cases in each group. Both groups were given raw rhubarb enema treatment, the reference group was given routine rehabilitation nursing measures, and the intervention group was given graded pain nursing measures on the basis of the reference group, after one day of admission and seven days of intervention, the changes of mental resilience, gastrointestinal function recovery and complications were compared between the two groups. Results After seven days of intervention, the scores of strength, optimism, resilience, and other psychological resilience in the two groups were higher than those before intervention, and those of the intervention group were higher than those of the reference group, the differences were statistically significant (P<0.05); the time of abdominal distension, abdominal pain, exhaust gas recovery, bowel sound recovery, and defecation in the intervention group were shorter than those in the reference group, the differences were statistically significant; the total complication rate in the intervention group was lower than that in the reference group, the difference was statistically significant(P<0.05). Conclusion The implementation of graded pain nursing measures in patients with acute pancreatitis treated with raw rhubarb enema is conducive to improving patients’ psychological anti-pressure ability, improving gastrointestinal functional indexes and reducing the incidence of complications.

Impact of posture management combined with family friendly voice wake-up service on intraoperative stress injury and recovery quality in patients undergoing general anesthesia surgery

Objective To observe and explore the impact of posture management combined with family friendly voice wake-up service on intraoperative stress injury and recovery quality in patients undergoing general anesthesia surgery. Methods A total of 110 patients undergoing general anesthesia surgery in the Nantong First People’s Hospital, Jiangsu Province from December 2021 to December 2022 were selected as the study objects, they were randomly divided into control group (supplemented by routine nursing and position management) and observation group (on the basis of the control group, it was supplemented with kinship voice wake-up service), with 55 cases in each group. The incidence of intraoperative stress injury, postoperative recovery quality, pain degree, and comfort level were compared between the two groups. Results The incidence of intraoperative pressure injury in the observation group was lower than that in the control group (P<0.05); the recovery room stay time, spontaneous breathing recovery time, eye opening time after exhalation, and caeheter withdrawal time in observation group were shorter than those in control group (P<0.05); the complications during anesthesia recovery in the observation group was lower than that in the control group (P<0.05); the postoperative visual analogue scale score of the observation group was lower than that of the control group, while the comfort scale score was higher than that of the control group (P<0.05). Conclusion Strengthening posture management and family friendly voice wake-up service for patients undergoing general anesthesia surgery can significantly reduce the risk of intraoperative stress injury, improve postoperative recovery quality, improve patients’ physical and mental comfort, promote prognosis, and have broad clinical application prospects.
