Discussion of mechanism of Compound Qiying Granules based on PERK/Nrf2 pathway in treating diabetic peripheral neuropathy

Objective To investigate the mechanism of Compound Qiying Granules on diabetic peripheral neuropathy (DPN) rats based on PERK/Nrf2 pathway. Methods A total of 72 SPF male Wistar rats weight 180-220 g and aged eight weeks were divided into normal control group, model group, positive drug group, and Compound Qiying Granules low-dose, medium-dose, high-dose groups by random number table method, with 12 rats in each group. Except the normal control group, the other five groups were continuously fed high-fat diet, and streptozotocin (30 mg/kg) was injected eight weeks later, and then rats with fasting blood glucose >11.1 mol/L were selected to continue high-fat diet for four weeks to build DPN model. After the DPN modeling was successful, Compound Qiying Granules low-dose, medium-dose, high-dose groups were given 1.17, 2.34, 4.68 g/kg Compound Qiying Granules, respectively, the positive drug group was given Trimethylamine Oxide (110 mg/kg), 2 ml/time, once a day, the normal control group and model group were given the same amount of double steaming water. After four weeks of intervention, blood glucose changes before and after intervention in the six groups were compared, and hematoxylin-eosin staining of sciatic nerve in the six groups was observed. Glucose regulatory protein 78 (GRP78), protein kinase RNA-like endoplasmic reticulum kinase (PERK), phosphorylated PERK (p-PERK), activating transcription factor 4 (ATF4), nuclear factor-erythroid 2-related factor 2 (Nrf2), C/EBP homologous protein (CHOP), cysteine aspartic acid specific protease-3 (caspase-3) in six groups of sciatic nerves were detected by Western blot. Results In the normal control group, the structure of myelinated nerve fibers was complete, the distribution was uniform, the arrangement was regular, the shape and structure of axons, myelin sheath, and nerve membranes were good and complete, and the shape and structure of endoneurals were normal. In the model group, there were loss and decrease of myelinated nerve fibers, partial disordered arrangement, partial axonal degeneration, segmental demyelination, thickening of nerve membrane, and hyperplasia of endoneural collagen fibers. The degree of degeneration, demyelination, thickening of nerve membrane, and hyperplasia of nerve intima in Compound Qiying Granules low-dose, medium-dose, high-dose groups were less than those in model group. After the successful DPN modeling, the blood glucose of the model group was higher than that of the normal control group (P<0.05), and there was no statistical significance in blood glucose of Compound Qiying Granules low-dose, medium-dose, high-dose groups compared with the model group (P>0.05). After four weeks of drug intervention, the blood glucose of model group was higher than that of normal control group, and those of Compound Qiying Granules low-dose, medium-dose, high-dose groups were lower than those of model group, and those of Compound Qiying Granules medium-dose, high-dose groups were lower than those in Compund Qiying Granules low-dose group (P<0.05). GRP78, p-PERK, ATF4, CHOP, and caspase-3 of sciatic nerve tissues in model group were higher than those in normal control group, and Nrf2 was lower than that in normal control group (P<0.05). GRP78, ATF4, and CHOP in Compound Qiying Granules medium-dose, high-dose groups were lower than those in model group, and Nrf2 were higher than those in model group; p-PERK and caspase-3 in Compound Qiying Granules high-dose group were lower than those in model group (P<0.05). ATF4 and CHOP of Compound Qiying Granules high-dose group were lower than those of Compound Qiying Granules low-dose group, CHOP of Compound Qiying Granules medium-dose group was lower than that of Compound Qiying Granules low-dose group (P<0.05). Conclusion Compound Qiying Granules has certain hypoglycemic and neuroprotective effects, and its mechanism may be to relieve endoplasmic reticulum stress by inhibiting PERK/Nrf2 pathway.

Effect of inflammatory exosomes derived from different synoviocytes on rabbit knee joint and its mechanism based on TLRs/NF-κB signal pathway

Objective To observe the effect of inflammatory exosomes derived from different synoviocytes on rabbit knee joint and its mechanism based on TLRs/NF-κB signal pathway. Methods Two synoviocytes were treated with lipopolysaccharide to induce inflammation, the exosomes were extracted separately. Twenty experimental rabbits were divided into five groups by random number table method: normal group, saline group, group A (injected with exosome suspension of A cell), group B (injected with exosome suspension of B cell), and group A+B (injected with two kinds of exosome mixed suspension), with four rabbits in each group. The normal group was not treated, the saline group was injected with normal saline, and the corresponding exosome suspension was injected into the joint cavity of rabbits in the exosome group (the concentration was 1×107/ml). After two weeks of intervention, the behavioral changes of rabbits in each group were recorded, and the expression levels of tumor necrosis factor α (TNF-α), interleukin-1β (IL-1β), IL-6 and matrix metalloproteinase 13 (MMP-13) in serum were detected by enzyme-linked immunosorbent assay, Toll-like receptor 2 (TLR2), P65, myeloid differentiation factor 88 (MyD88) and TNF receptor-associated factor 2 (traf2) were detected by Western blot. Results There was no significant difference in behavioral Lequesne MG index between normal group and saline group before and after intervention (P>0.05). The behavioral Lequesne MG index of rabbits in groups A, B and A+B after intervention was higher than that before intervention, and higher than that in normal group (P<0.05); the levels of TNF-α, IL-1β, IL-6, and MMP-13 in serum and the expressions of TLR2, P65, MyD88, and traf2 in synovium tissue of rabbits in groups A, B and A+B were higher than those in normal group (P<0.05); the levels of TNF-α, IL-1β, IL-6, and MMP-13 in serum of rabbits , and the levels of TLR2, P65, MyD88, and traf2 in synovium in group A+B were higher than those in groups A and B (P<0.05). The protein expressions of TLR2, P65, MyD88, and traf2 in synovium tissue of rabbits in group A were higher than those in group B (P<0.05). Conclusion Both kinds of synoviocytes secretions in inflammatory environment can activate synovial TLRs/NF-κB signal pathway aggravates synovial inflammation.

Effect of CD4+Foxp3+ regulatory T cell on the osteogenic differentiation of bone marrow mesenchymal stem cells

Objective To investigate the effect of CD4+Foxp3+ regulatory T cell on the osteogenic differentiation of bone marrow mesenchymal stem cell (BMMSC). Methods The experiment was divided into three groups, the control group consisted of CD4+ T cells and BMMSC; the experimental group consisted of regulatory T cells and BMMSC; the inhibitory group consisted of regulatory T cells and BMMSC and added GW4869 (exosome inhibitor). Mineralization induction was performed in all three groups. ALP expression and activity were detected by alkaline phosphatase (ALP) staining and activity at day 0 and 7, mineralization nodules formation and calcium content were detected by alizarin red staining and calcium content analysis at day 0 and 21, levels of bone differentiation genes ALP, OCN, and Runx2 mRNA were detected by PCR at day 7. Results the ALP staining area and ALP activity in the experimental group were higher than those in the control group (P<0.01). The formation and calcium content of mineralized nodules stained with alizarin red were higher than those in the control group (P<0.01), and the expression levels of ALP, OCN, and Runx2 mRNA were higher than those in the control group (P<0.01). The ALP staining area and ALP activity in the inhibitory group were lower than those in the experimental group (P<0.01), the formation and calcium content of mineralized nodules stained with alizarin red were lower than those in the experimental group (P<0.01), and the expression levels of ALP, OCN, and Runx2 mRNA were lower than those in the experimental group (P<0.01). Conclusion Tregs could promote the osteogenic differentiation of BMMSC, and Tregs exosomes may participate in this process.

20
Construction and validation of prognostic model in patients with postoperative early-onset colorectal cancer

Objective To investigate the independent prognostic factors of postoperative early-onset colorectal cancer (EOCRC) and to establish a prognostic nomogram model. Methods Patients diagnosed with EOCRC and treated surgically during 2010—2015 were collected from the SEER database and randomly divided into a training set and a validation set in a ratio of 7∶3. Independent prognostic factors were screened by Cox regression analysis, and the nomogram was constructed. The accuracy and reliability of the prognostic model were evaluated by C-index, receiver operating characteristic (ROC) curve, calibration chart, and decision curve analysis (DCA). Results Cox regression analysis showed that race, marital status, histological type, TNM stage, differentiation degree, SEER stage and carcinoembryonic antigen level were independent prognostic factors that ultimately affected the survival of patients (HR>1, P<0.05). The C-index of training set and verification set were 0.801 and 0.811, respectively. In the training set, the 3-year and 5-year survival rates predicted by the model were 0.862 and 0.845, respectively. In the validation set, the model predicted a 3-year and 5-year AUC of 0.860 and 0.854, respectively. The calibration curve and decision curve indicated that the prognostic model can accurately predict the survival of patients. Conclusion In this study, independent prognostic factors in postoperative EOCRC patients are identified and a clinically available prognostic nomogram model is established.

Discussion on the medication rule of Professor Feng Ling in treating chronic heart failure based on data mining

Objective To investigate the medication rule of Professor Feng Ling in the treatment of chronic heart failure. Methods Effective chronic heart failure medical records of Professor Feng Ling’s outpatient department in Guang’anmen Hospital, China Academy of Chinese Medical Sciences from January 2016 to January 2022 were collected. Traditional Chinese medicine inheritance computing platform (V3.0) was used to conduct drug frequency, four nature, five flavours, channel tropism, efficacy, and association rules of prescriptions, and to explore medication rules. Using the “prescription cluster analysis” module, the core prescription was mined by the entropy hierarchical clustering method of complex system. Results A total of 1 151 effective prescriptions were collected, involving 225 kinds of Chinese materia medica. There were ten kinds of Chinese materia medica with frequency≥467, among which Astragali Radix, Santali Albi Lignum, and Mori Cortex had the highest frequency. In the four nature, cold and warm were the main, followed by flat. In the five flavours, sweet and pungent were the main, followed by bitter. In the channel tropism, spleen, lung, and heart channels were the main. The efficacy of the drug was divided into 17 categories, such as regulating qi, tonifying deficiency, diuresis and diffusing dampness, promoting blood circulation for removing blood stasis, resolving phlegm and relieving cough and asthma, and so on. Association rule analysis obtained 18 associated couplet medicines. Eight core prescription were obtained by high-frequency drug cluster analysis. The core presicripton were composed of Astragali Radix, Mori Cortex, Arecae Pericarpium, Poria, Polyporus, Salviae Miltiorrhizae Radix Et Rhizoma, Santali Albi Lignum, and Aucklandiae Radix. Conclusion Professor Feng Ling’s medication approach for chronic heart failure involves treating qi deficiency, blood stasis, and water stasis as the core pathogenesis. The treatment of invigorating spleen and benefiting qi, activating blood circulation to dissipate blood stasis, inducing diuresis for removing edema as the method. “Spleen, lung and heart” are treated simultaneously; “sweet, pungent, and bitter” are used all at once.

Analyses of chemical constituents and components absorbed into blood of Yiqi Shengjin Bag Tea by HPLC-Q-TOF-MS

Objective To identify the main chemical constituents and components absorbed into blood of Yiqi Shengjin Bag Tea by HPLC-Q-TOF-MS technology. Methods Six eight-week-old SPF male SD rats, weighing 280-300 g, were divided into administration group and control group by random number table method, with three rats in each group. In administration group, 4 ml of Yiqi Shengjin Bag Tea water extract was intragastric (the crude drug concentration was 1 g/ml), while in blank group, 4 ml of normal saline was intragastric, twice a day for continuous administration, and blood samples were collected from abdominal aorta 2 h after administration on the morning of the fourth day to prepare serum samples. A Waters Xselect HSS T3 C18 chromatographic column (50 mm×2.1 mm, 2.5 μm) was used to collect the chemical components of Yiqi Shengjin Bag Tea water extract and serum samples under positive and negative ion modes; gradient elution was performed with 0.1% formic acid-acetonitrile as mobile phase. Results Seventeen compounds were identified from Yiqi Shengjin Bag Tea water extract, mainly ginsenosides. Meanwhile, eight chemical constituents were identified from the serum sample, which were betaine, ginsenoside Rg1, ginsenoside Re, harpagoside, pseudoginsenoside F11, ginsenoside Rb1, ginsenoside Rd, and erianin. Conclusion HPLC- Q-TOF-MS technology was used to analyze the main chemical components of Yiqi Shengjin Bag Tea, which can help to reveal pharmacodynamic material basis of Shengjin Bag Tea, and provide reference for further quality control, pharmacology, and activity mechanism research.

Research progress of cell autophagy in non-small cell lung cancer

Lung cancer is a malignant tumor with a high mortality rate, with non-small cell lung cancer (NSCLC) accounting for over 85% of lung cancer. The treatments for NCSLC mainly include surgery, radiotherapy, chemotherapy,immunotherapy, and so on. At present, the therapeutic effects of radiotherapy and chemotherapy on lung cancer are not satisfactory. Immunotherapy is only effective for specific populations and is prone to drug resistance. Therefore, there is an urgent need to develop new NSCLC treatment methods. Autophagy has been reported to be one of the important factors affecting the occurrence and development of NSCLC. This article analyzes the double-edged sword role of autophagy in tumors, reviews the role of autophagy in influencing the efficacy, sensitivity and drug resistance of chemotherapy, radiotherapy, and immunotherapy for NSCLC, and provides clues for the development of new treatment strategies for NSCLC and the discovery of new biomarkers.

Research progress of T cell subsets in the process of pulmonary fibrosis

Pulmonary fibrosis is a chronic interstitial disease characterized by changes in lung tissue structure, and idiopathic pulmonary fibrosis is the most serious one. The etiology of idiopathic pulmonary fibrosis is unknown and the pathogenesis is complex, with T lymphocyte-dominated immune inflammation playing a key role in the lung fibrosis process. T cells are classified into different subpopulations according to their different pathways in immune inflammation: Th cells, Treg cells, Tc cells, and NKT cells. Different T lymphocyte subsets and cytokines mediated by them are highly expressed in pulmonary fibrosis, which can regulate immune injury and inflammatory reaction in pulmonary fibrosis by releasing or inhibiting inflammatory cytokines, and then affect the degree of lung tissue injury, collagen secretion and myofibroblast proliferation, so as to participate in the process of pulmonary fibrosis. In this article, the different pathological responses of T cell subsets and their induced cytokines in the process of pulmonary fibrosis are reviewed, and the mechanisms of their action in the process of pulmonary fibrosis is discussed, which could provide a deeper direction for the clinical treatment and basic research of pulmonary fibrosis.

Research progress on correlation between aldehyde dehydrogenase 2 gene polymorphism and the efficacy of nitroglycerin in patients with stable angina pectoris

Mitochondrial acetaldehyde dehydrogenase 2 (ALDH2) has multiple gene polymorphism loci, the most common mutation loci is rs671, which is associated with the incidence and severity of coronary artery disease. ALDH2 gene mutation can lead to a significant decrease in its enzyme activity, and angina pectoris patients with ALDH2 gene mutation are more likely to develop tolerance during nitroglycerin administration. Therefore, the relationship between ALDH2 gene mutation and nitroglycerin tolerance is the focus of current research, and the development of ALDH2 enzyme activity activators may become a new treatment method to improve nitroglycerin tolerance. This article reviews the correlation between ALDH2 gene polymorphisms and the efficacy of nitroglycerin in patients with coronary heart disease, in order to provide a new idea and basis for clinical treatment of stable angina pectoris of coronary heart disease.

Research progress of vitamin D supplementation in the treatment of inflammatory bowel disease

Vitamin D not only plays a classic physiological role in maintaining calcium and phosphorus balance, but its immune regulatory role in the pathophysiological process of inflammatory bowel disease is also receiving increasing attention. Previous studies have shown that vitamin D deficiency is very common in patients with inflammatory bowel disease, and vitamin D supplementation plays an important role in reducing excessive inflammatory response and improving the prognosis of patients with inflammatory bowel disease. However, there is currently no unified standard for the dosage form, dosage, and supplementation goals of vitamin D supplementation preparations. This review will review recent researches on inflammatory bowel disease and vitamin D, providing clinical reference for supplementing vitamin D in patients with inflammatory bowel disease.

Research progress of severe analgesia and sedation on brain protection in patients with neurological severe conditions

Traumatic brain injury (TBI) patients with pain, agitation and stress response lead to tachycardia, elevated blood pressure, and increased increased intracranial pressure, etc., increasing the risk of rebleeding, accidental injury, and death. Effective analgesia and sedation help control intracranial pressure and avoid violent fluctuations, especially during invasive procedures. Analgesic sedation can achieve brain protection. There is no definite plan of analgesia and sedation for patients with severe neurological diseases. How to rational analgesia and sedation is a problem faced by clinicians. This article reviewed the pathophysiological changes, the necessity of analgesia and sedation, the protective effects of commonly used analgesia and sedation drugs, and related debates, aiming to provide reference for the selection of analgesia and sedation programs for craniocerebral trauma.

Clinical research progress of Chinese medicine in the treatment of interstitial cystitis

Interstitial cystitis is a chronic non-bacterial cystitis of unknown origin, which is difficult to treat clinically, with recurrent episodes of urinary frequency and painful symptoms that are difficult to heal and seriously affect the physical and mental health and quality of life of patients. Traditional Chinese medicine believes that interstitial cystitis is located in the bladder and is closely related to the spleen, kidney, and liver. The pathogenesis may be caused by qi and blood stasis obstruction, dampness and heat stagnation, spleen and kidney deficiency. Its characteristics may be the mutual influence of “deficiency, blood stasis, dampness, and stagnation” or a combination of pathogenic factors. Traditional Chinese medicine therapy, as one of the important treatment methods for this disease, pays attention to the methods of promoting blood circulation and soothing the liver, clearing heat and promoting diuresis, and strengthening the spleen and kidney when using medication. In addition to oral medication, acupuncture and moxibustion and traditional Chinese medicine infusion can be used to improve the clinical efficacy of the disease.

Research progress in the relationship between maternal near miss and mental disorders

Maternal near miss (MNM) have become the focus of maternal health research in recent years and efforts have been made to reduce the incidence of MNM worldwide. MNM events cause serious harm to the physiology of pregnant women, resulting in a series of negative emotions and even more prone to mental disorders. This paper comprehensively discusses the negative effects of MNM events, the correlation between MNM and mental disorders and the research progress of corresponding intervention measures, aiming to provide theoretical basis and method reference for reducing the long-term impact of severe complications on the mental psychology of pregnant women. Meanwhile, it emphasizes the importance of multi-faceted joint intervention to control the adverse emotions as soon as possible to reduce the risk of mental disorders.

Research progress on mechanism of action of Rhizoma Acori Tatarinowii in prevention and treatment of Alzheimer’s disease

Alzheimer’s disease (AD) is the most common cause of progressive cognitive dysfunction in the elderly. In recent years, Rhizoma Acori Tatarinowii has become a research hotspot at home and abroad because of its unique advantages. At present, the mechanism of Rhizoma Acori Tatarinowii improving cognition has not been thoroughly studied, and it may act on multiple targets simultaneously, thereby protecting neurons, improving synaptic remodeling, regulating central cholinergic system, inhibiting neuroinflammation, reducing oxidative stress, and improving intestinal microecosystem. In order to provide reference for clinical and experimental studies, this paper systematically summarizes the mechanism of action of Rhizoma Acori Tatarinowii in preventing and treating AD in recent years.

Survey of Chinese rheumatologists’ attitude towards traditional Chinese medicine for Sjögren’s syndrome

Objective To understand the attitude of Chinese rheumatologists towards the treatment of Sjögren’s syndrome (SS) with traditional Chinese medicine, and to analyze the problems facing the application of traditional Chinese medicine. Methods An anonymous questionnaire survey was used to investigate the attitudes of Chinese rheumatology clinicians attending the 2022 “Fengyun Forum” on the treatment of SS by traditional Chinese medicine. Results A total of 347 questionnaires were distributed, and 320 were effectively collected, with an effective recovery rate of 92.22%. The vast majority of doctors affirmed the efficacy of traditional Chinese medicine and hoped that it could relieve the symptoms of dryness (88.41%) and fatigue (68.12%) in SS patients. Logistic regression analysis showed that the elderly with Chinese medicine education background and working years were more inclined to recommend Chinese medicine treatment for SS (P<0.05). Most doctors (65.63%) thought that the proportion of clinical application of Chinese medicine was less than 40%, which was related to the lack of Chinese medicine guidelines and too few clinical studies. Conclusion Chinese doctors pay great attention to the advantages and characteristics of traditional Chinese medicine, but the lack of guidelines and evidence-based evidence restrict the application and development of traditional Chinese medicine. It is urgent to formulate SS guidelines for traditional Chinese medicine diagnosis and treatment and strengthen the research of traditional Chinese medicine.

Objective To analyze epidemiology data of patients with acute carbon monoxide poisoning (ACOP) in the Inner Mongolia Autonomous Region People’s Hospital (“our hospital” for short) from 2019 to 2021, so as to provide the basis for the prevention and control of ACOP in this area. Methods ACOP patients admitted to our hospital from January 2019 to December 2021 were analyzed by gender, age, occurrence time, occurrence place, cause of poisoning, and severity of poisoning. Results During this period, our hospital treated 921 patients with ACOP, age ranged from 5 months to 89 years, with a median age of 51.00 (35.00, 63.00) years. The number of poisoning in heating season was larger than that in non-heating season, and the difference was statistically significant (P<0.05). There was statistical significance in the causes and places of poisoning of ACOP patients in different years (P<0.05). There was statistical significance in the incidence of disease in different age and poisoning degree grades (P<0.05). Conclusion This study provides the basic data of ACOP patients in our hospital, clarifies the epidemiological characteristics of ACOP in this region, and provides a reference for the prevention and treatment of ACOP.

Application of respiratory case discussion based on picture archiving and communication system in the practice of master students

Objective To explore the application of respiratory case discussion based on picture archiving and communication system (PACS) in the practice of master students. Methods From January 2021 to June 2022, a total of 105 graduate students with professional degrees in the Department of Respiratory and Critical Care Medicine of Shandong University of Traditional Chinese Medicine were selected, and 15 items were set up in the questionnaire. The observation indicators were the familiarity of the trainee students with PACS, the enthusiasm of participating in the discussion of respiratory cases based on PACS, and the recognition of learning effect. Each project had four options reflecting different levels of familiarity or recognition. Results A total of 105 questionnaires were sent out in this survey, and 105 were effectively collected, with an effective recovery rate of 100%. The degree of cognition of the application of PACS in the diagnosis of respiratory diseases was poor, and the proportion of people who were very familiar with PACS was 26.67% to 42.86%. The participation in the discussion of respiratory cases based on PACS was higher, and the recognition of the application effect of PACS in the review of radiography and the discussion of respiratory cases was higher, with the proportion of students who were highly approved + approved ranging from 80.98% to 91.43%. Conclusion Case discussion based on PACS is a good teaching method that can increase students’awareness of discussing diseases. In order to make more effective and reasonable use of this teaching method, it is necessary to take the teachers and clinical graduate students to participate in and make efforts.

Analysis on mediating effect of self-concept on subjective well-being and social support in patients with breast cancer after operation

Objective To observe the subjective well-being and social support level of patients with breast cancer after operation, and to analyze the mediating effect of self-concept between the two in patients. Methods A total of 100 postoperative breast cancer patients admitted to West China Hospital of Sichuan University from October 2019 to October 2021 were selected as the study objects. Self-concept (Tennessee self-concept questionnaire [TSCS]), subjective well-being (Memorial University of Newfoundland scale of happiness [MUNSH]), and social support (social support rating scale [SSRS]) were assessed a month after surgery. The relationship between self-concept, subjective well-being, and social support of patients after breast cancer surgery were observed, and the mediating effect of self-concept on subjective well-being and social support of patients after breast cancer surgery were analyzed. Results A total of 100 questionnaires were collected with an effective recovery rate of 100.00%. The scores of TSCS, MUNSH, and SSRS of 100 patients after breast cancer surgery were in the medium level. The Pearson correlation test showed that self-concept was positively correlated with subjective well-being and social support (r>0, P<0.05). Through the analysis of mediating effects, the equation of mediating effects can be obtained as follows: Y=0.458X+0.384M+e3(Y: subjective well-being; X: social support; M: self-concept). The results showed that the level of social support could indirectly affect the subjective well-being of patients through the self-concept of patients, which was a partial mediating effect, and the contribution rate of the mediating effect to the total effect was 41.72%. Conclusion Social support after breast cancer surgery can not only affect patients’ subjective well-being through self- concept, but also directly affect patients’ self-well-being.

Effect of Insulin Degludec/Insulin Aspart combined with Liraglutide on glucose and lipid metabolism and visceral adiposity index in overweight/obese patients with type 2 diabetes mellitus

Objective To investigate the effects of Insulin Degludec/Insulin Aspart and Liraglutide (IDAL) on glucose and lipid metabolism and visceral adiposity index in newly diagnosed overweight/obese patients with type 2 diabetes mellitus. Methods A total of 112 newly diagnosed overweight/obese type 2 diabetes mellitus patients who were treated in the Department of Endocrinology, Quanzhou First Hospital Affiliated to Fujian Medical University from May 2021 to June 2022 were divided into IDAL group and bolus-basal Insulin (BBI) group by random number table method, with 56 cases in each group. The two groups were intensive treated for eight weeks. The glucose and lipid metabolism indexes, insulin resistance related indexes, visceral adiposity index, the adverse reactions, and medication compliance were compared before and after treatment. Results After treatment, fasting blood glucose, 2 h postprandial blood glucose, glycated hemoglobin, fructosamine, triglyceride, total cholesterol, and low density lipoprotein cholesterol in both groups were lower than before treatment, while high density lipoprotein cholesterol was higher than before treatment, and triglyceride, total cholesterol and low density lipoprotein cholesterol in the IDAL group were lower than those in the BBI group, high density lipoprotein cholesterol was higher than that in BBI group (P<0.05). After treatment, body mass index, waist circumference and visceral fat index in the IDAL group were lower than before treatment, and the IDAL group was lower than the BBI group (P<0.05), and the steady-state model insulin resistance index in the two groups was lower than before treatment, and the IDAL group was lower than the BBI group (P<0.05). The incidence of gastrointestinal adverse reaction in IDAL group was higher than that in BBI group, and the incidence of hypoglycemia in the IDAL group was lower than that in the BBI group, while the drug compliance was significantly higher (P<0.05). Conclusion IDAL intensive therapy can effectively improve the glucose metabolism indexes of overweight/obese patients with type 2 diabetes, and has more advantages in the improvement of lipid metabolism indexes, insulin resistance and visceral adiposity index, and has a lower incidence of hypoglycemia and better medication compliance.

Objective To investigate the prognostic value of serum osteosin (OSTN) combined with procollagen typeⅠN-terminal propeptide (PⅠNP) in predicting hip dysfunction after proximal femoral nail antirotation (PFNA) in elderly pa- tients with intertrochanteric fracture of femur (IFF). Methods A total of 200 elderly IFF patients who underwent PFNA surgery in the Department of Orthopedics of Yellow River Sanmenxia Hospital, Henan Province from February 2019 to December 2021 were selected and divided into adverse group (51 cases) and excellent group (149 cases) according to the Harris hip function score at one year after surgery. The clinical data and serum OSTN, PⅠNP levels were compared between the two groups, and the influencing factors of hip dysfunction in elderly IFF patients after PFNA surgery were analyzed. The predictive value of serum OSTN and PⅠNP levels in elderly IFF patients after PFNA surgery was analyzed by receiver operating characteristics (ROC) curve. Results Age, proportion of complications before surgery, proportion of American Society of Anesthesiologists rating ≥Ⅲ before surgery, proportion of Evans-Jensen classification ≥Ⅲ before surgery, proportion of severe osteoporosis, and serum OSTN, PⅠNP levels in the adverse group were higher than those in the excellent group, and the differences were statistically significant (P<0.05). The results of multivariate analysis showed that age, preoperative comorbidities, American Society of Anesthesiologists rating before surgery, Evans-Jensen classification before surgery, osteoporosis, and serum OSTN, PⅠNP levels were independent influencing factors for hip dysfunction in elderly IFF patients after PFNA surgery (P<0.05). ROC curve analysis results showed that the area under the curve of the combined prediction of serum OSTN and PⅠNP levels of hip dysfunction in elderly IFF patients after PFNA was larger than that of the two alone (P<0.05). Conclusion The elevated serum OSTN and PⅠNP levels in elderly IFF patients are associated with hip dysfunction after PFNA surgery, and may be auxiliary predictors of IFF.

Influencing factors of intraoperative pressure injury in patients with thoracolumbar fracture

Objective To investigate the influencing factors of intraoperative pressure injuries (PI) in patients with thoracolumbar fracture. Methods The clinical data of 150 patients undergoing prone thoracolumbra fracture surgery in the Shanghai Jiao Tong University Medical College Affiliated Xinhua Hospital from January to July 2022 were retrospectively analyzed. According to whether PI occurred during the operation, the patients were divided into the occurrence group (24 cases) and the non-occurrence group (126 cases). Univariate statistical analysis was performed on the general information and clinical data of the two groups, and the indexes that may affect the intraoperative PI of the patients undergoing thoracolumbar fracture surgery were initially screened. The independent risk factors of PI in patients undergoing thoracolumbar fracture surgery were analyzed by logistic regression. Results The ratio of PI injury in head and face surgery was the highest. Preoperative serum albumin (Alb) level, operation time, protective materials used during operation, and vasoactive drugs were compared between the two groups, the differences were statistically significant (P<0.05). Logistic regression analysis showed that low serum Alb expression before surgery, long operation time, protective materials used during surgery, and vasoactive drugs were the influencing factors for intraoperative PI in patients with thoracolumbar fracture surgery (P<0.05). Conclusion The low expression of serum Alb before surgery, long operation time, protective materials used during operation, and vasoactive drugs may affect the occurrence of PI in patients undergoing thoracolumbar fracture surgery.

Application of resistance training based on behavior change theory in rehabilitation of elderly patients with lower extremity fracture

Objective To explore the application of resistance training based on behavior change theory in rehabilitation of elderly patients with lower extremity fracture. Methods A total of 96 elderly patients with lower limb fracture treated in Xinhua Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from April 2021 to April 2022 were included and divided into observation group (48 cases) and control group (48 cases) by random number table method. The control group received conventional resistance training intervention, and the observation group received resistance training intervention based on behavior change theory. After four weeks of intervention, flexor muscle strength and extensor muscle strength were compared between the two groups before and after intervention. The lower extremities function scale (LEFS) scores of the two groups were compared before intervention and after three months of follow-up, and the physical self-maintenance scale (PSMS) scores of the two groups were compared at the time of discharge and three months of follow-up, and the incidence of complications was recorded in two groups. Results After four weeks of intervention, the flexor muscle strength and extensor muscle strength in both groups were higher than those before intervention, and those of observation group were higher than those of control group (P<0.05). After three months of follow-up, LEFS scores in both groups were higher than those before intervention, and those of observation group were higher than those of control group (P<0.05). After three months of follow-up, the PSMS scores of both groups were lower than those at discharge, and those of observation group were lower than those of control group (P<0.05). There was no significant difference in the total incidence of complications between the two groups (P>0.05). Conclusion Resistance training based on behavior change theory can improve lower limb function, muscle strength, and self-care ability in elderly patients with lower limb fracture.

Application of early cognitive language rehabilitation training combined with rTMS in rehabilitation of children with global developmental delay

Objective To observe the application of early cognitive language rehabilitation training combined with repetitive transcranial magnetic stimulation (rTMS) in children with global developmental delay (GDD). Methods A prospective randomized controlled trial was used, 64 children newly diagnosed with GDD in Hebei Children’s Hospital from January 2021 to March 2022 were selected as study objects. They were divided into the experimental group and the control group according to the random number table method, with 32 cases in each group. The control group used early cognitive language rehabilitation training intervention, and the experimental group was combined with rTMS intervention on the basis of the control group, the intervention lasted nine months. The intelligence level, neuropsychobehavioral development, motor function, and muscle tension of the two groups were compared before and after intervention. Results During intervention, four cases were excluded, and two cases were lost to follow-up in the control group; in the experimental group, one case was transferred to hospital for treatment and one case was lost to follow-up; finally, 30 patients in both groups were followed up. After intervention, the verbal intelligence quotient, performance intelligence quotient, and full intelligence quotinent scores of the two groups were higher than those before intervention, and the experimental group was higher than the control group (P<0.05). After intervention, the scores of adaptive developer and gross motor function measure-88 in the two groups were higher than those before intervention, and those in the experimental group were higher than those in the control group (P<0.05). After intervention, the muscle tension of the two groups was better than that before intervention, and the experimental group was better than the control group (P<0.05). Conclusion Early cognitive language rehabilitation training combined with rTMS intervention can significantly improve the intelligence level of children with GDD, and can also improve muscle tone, thereby improving motor function.

Systematic evaluation of serum vitamin D levels and efficacy of vitamin D supplementation in children with atopic dermatitis

Objective To determine serum 25-hydroxyvitamin D (25[OH]D) levels in children with atopic dermatitis (AD), and the effect of vitamin D supplementation on AD was evaluated by systematic analysis of vitamin D intervention. Methods The relevant literature on vitamin D and AD and vitamin D therapy for AD were searched by computer from PubMed, EMbase, CNKI, Wanfang Academic Journal Database, and VIP from January 2010 to November 2022. A meta-analysis method was used to systematically evaluate the retrieved literatures that met the inclusion criteria. Results Five randomized controlled trials (RCT) and 15 case-control studies were included. In RCT, eczema area and severity index in experimental group was higher than that in control group(WMD=-3.79, 95%CI[-6.38,-1.20], P=0.004); the scoring of atopic dermatitis in experimental group was higher than that in control group (WMD=-6.06, 95%CI[-10.12,-2.00], P=0.003). In the case- control study, serum 25(OH)D levels in AD children group were lower than that in control group (SMD=-0.80, 95%CI [-1.18,-0.41], P<0.000 1). Conclusion Children with AD are at high risk of vitamin D deficiency, and vitamin D supplementation can improve clinical symptoms in children with AD.

Effects of ultrasound parameters and serum fibrinogen, D-dimer on the occurrence of lower extremity deep venous thrombosis after cesarean section

Objective To investigate the effects of ultrasound parameters and serum fibrinogen (FIB) and D-dimer (D-D) on the occurrence of lower extremity deep venous thrombosis (DVT) after cesarean section. Methods The clinical data of 82 women who underwent cesarean section in Suzhou Integrated Traditional and Western Medicine Hospital of Jiangsu Province from March 2019 to March 2022 were retrospectively analyzed. They were divided into DVT group and uncomplicated group according to whether postpartum DVT was concurrent or not. The general data, ultrasound parameters (peak systolic velocity [PSV], resistance index [RI], and pulsatility index [PI]) and serum FIB, D-D levels between two groups were analyzed; logistic regression analysis was used to analyze the influencing factors of DVT after cesarean section; receiver operating characteristic (ROC) curve was used to analyze the diagnostic value of ultrasound parameters PSV, RI, PI combined with serum FIB and D-D in DVT after cesarean section. Results DVT occurred in 22 of 82 parturients after cesarean section. The proportion of hyperlipidemia history, thrombosis history, and RI, PI, D-D, and FIB in DVT group were higher than those in uncomplicated group, and PSV was lower than that in uncomplicated group (P<0.05). Hyperlipidemia history, thrombosis history, PSV, RI, PI, D-D, and FIB were the influencing factors of DVT after cesarean section (P<0.05). The area under the curve of ultrasonic parameters PSV, RI, PI combined with serum D-D and FIB in the diagnosis of DVT after cesarean section were larger than each single test. Conclusion The ultrasonic parameters PSV of DVT patients after cesarean section decreased, RI and PI increased, and serum D-D and FIB levels increased. The combined test has higher diagnostic value.

Correlation of serum vitamin D level with insulin resistance and inflammatory response in pregnant women

Objective To explore the changes of serum vitamin D (Vit D) level and its correlation with insulin resistance (IR) and inflammatory response in pregnancy. Methods A total of 53 pregnant women with gestational diabetes mellitus (GDM) were selected as experimental group from the Xishan People’s Hospital of Wuxi, Jiangsu Province (hereinafter referred to as “our hospital”) from January 2020 to December 2021, another 53 normal pregnant women in our hospital during the same period were selected as control group. The clinical data in both groups were recorded; the levels of fasting blood glucose (FBG), fasting insulin (FINS), Homeostasis model evaluation method-insulin resistance (HOMA-IR), total cholesterol (TC), triglyceride (TG), 25-hydroxyvitamin D (25[OH]D), hypersensitive C-reactive protein (hs-CRP), tumor necrosis factor-α (TNF-α), and interleukin-6 (IL-6) in both groups were compared; the correlation between serum 25(OH)D and each index was analyzed by Pearson correlation analysis. Results The level of 25(OH)D in experimental group was lower than that in control group, the levels of FBG, FINS, TG, HOMA-IR, hs-CRP, TNF-α, and IL-6 in experimental group were higher than those in control group, and the differences were statistically significant(P<0.05). The level of serum 25(OH)D was negatively correlated with FBG, FINS, TG, HOMA-IR, CRP, TNF-α, and IL-6(r<1,P<0.05). Conclusion Compared with healthy pregnant women, the level of Vit D in GDM pregnant is significantly decreased, which may be one of the important factors causing inflammation, IR and abnormal glucose and lipid metabolism, etc. Clinical attention should be paid to the changes of Vit D level in GDM pregnant.

Study on bonding strength, aging resistance, and microleakage of bulk filling resins and dentin

Objective To evaluate the bonding strength, aging resistance, and microleakage between different bulk filling resins and dentin. Methods A total of 140 unified vitro molars were chosen(from Department of Stomatology in China Rehabilitation Research Center), 120 of which had their dentin layer revealed and covered with adhesives. They were divided into four groups by random number table method, with 30 cases in each group. The four groups were whole filled with FiltekTM Bulk Fill resin (group A), X-tra Bulk Fill resin (group B), SureFil SDR flowable resin (group C), and stratified filled with Filtek Z350 conventional resin (group D), the four groups were divided into two subgroups by random number table method, with 15 cases in each group. The first subgroup was after filling, that was before aging; the other subgroup underwent 10 000 temperature cycles of aging, that was after aging, shear strength tests were performed. The difference of bonding strength between different resin materials and before and after aging was compared by single factor analysis of variance. The remaining 20 molars were prepared into a Class Ⅰ cavity with a depth of 4 mm, with five molars in each group, filled and aged, soaked in 2% methylene dye for 24 h, cut into 1 mm slices, and the dye infiltration at the edge of the filling body was observed under a type microscope. Results Before aging, the bonding strength of group C was higher than that of groups A, B, and D, and the differences were statistically significant (P<0.05). After aging, the bond strength of all groups decreased compared with that before aging, and the bond strength of group C was higher than that of groups A, B, and D, the differences were statistically significant (P<0.05). There was no significant difference in interface microleakage scores among all groups (P>0.05). Conclusion SDR with low viscosity performed well in the trial, having a better bonding effect than conventional resin.

Clinical effect of abdominal acupuncture combined with pelvic floor muscle training in the treatment of poststroke constipation

Objective To observe the clinical effect of abdominal acupuncture combined with pelvic floor muscle training in the treatment of post stroke constipation. Methods Ninety patients with poststroke constipation admitted to the First People’s Hospital of Zhangjiagang, Jiangsu Province from May 2019 to July 2021 were selected and they were divided into control group A (pelvic floor muscle training), control group B (abdominal acupuncture), and study group (abdominal acupuncture combined with pelvic floor muscle training) by random number table method, with 30 cases in each group. The traditional Chinese medicine symptom score, gastrointestinal function, modified Rankin Scale (mRS), and patient-assessment of constipation quality of life scale (PAC-QOL) were compared before and after treatment among the three groups. Results After treatment, the traditional Chinese medicine symptom scores of the three groups were lower than those before treatment, and the study group was lower than control group A and control group B (P<0.05). After treatment, the resting anal pressure of the three groups was higher than that before treatment, and the study group was higher than that of control group A and control group B (P<0.05). After treatment, mRS scores and PAC-QOL scores of the three groups were lower than those before treatment, and the study group was lower than control group A and control group B (P<0.05). Conclusion Abdominal acupuncture combined with pelvic floor muscle training in the treatment of poststroke constipation has definite clinical effect, which can improve the neurological function, gastrointestinal function and quality of life of patients.

Under the background of traditional Chinese medicine culture going globally, the translation of traditional Chinese medicine terms with a variety of meanings has become a critical study. However, the research on translation of traditional Chinese medicine terms such as the ones with Chinese character “Jia” is still insufficient. Based on 5W Communication Model, this paper compares English versions of traditional Chinese medicine terms with Chinese character “Jia” in Treatise on Cold Pathogenic Diseases translated by Liu Guohui and Li Zhaoguo, and explores issues including the basic quality of translators, translation methods applied in translated text, the range of target recipients, channels of transmission and effects of translation which are all supposed to be attached more attention during the process of intercultural communication, aiming at providing references for translation study and practice of traditional Chinese medicine terms and supporting traditional Chinese medicine culture “going globally”.

Professor Zhang Fuli’s experience in treating Hashimoto thyroiditis based on taiyin-yangming theory

Hashimoto thyroiditis is a clinically common autoimmune disease with a complex pathogenesis. Western medicine uses surgery, thyroid hormone replacement therapy, immunosuppressive drugs and so on to relieve symptoms and delay the course of the disease, with significant adverse effects. This disease belongs to the category of “goiter disease” in Chinese medicine. Physical, dietary, and emotional factors are related to the occurrence of this disease, and the pathogenesis of the disease is mostly due to the deficiency of vital qi and the stagnation of evil, the stagnation of phlegm and the loss of liver circulation. Pathological factors are mostly qi stagnation, phlegm coagulation, and blood stasis. Professor Zhang Fuli believes that the basic pathogenesis of the disease is dampness abundance due to spleen deficiency, with dryness and, dampness causing the disease together. Dryness and dampness interact throughout the course of the disease. Based on the taiyin-yangming theory, Professor Zhang Fuli proposes the basic treatment principle of moistening dryness, resolving dampness, and regulating the spleen and stomach, and draft the foundation plan by oneself. During the clinical treatment, according to the different stages of the disease with excessive dryness or severe dampness, the moisturizing or dispelling power of various drugs in the formula should be increased or decreased, in accordance with the growth and decline of yin and yang in the four seasons, and the therapeutic effect should be quite good with the addition and subtraction according to the symptoms.

Effect of percutaneous acupoint electrical stimulation combined with acupuncture assisted rehabilitation training for patients with hemiplegia after stroke

Objective To explore the effect of percutaneous acupoint electrical stimulation combined with acupuncture assisted rehabilitation training for patients with hemiplegia after stroke. Methods A total of 136 patients with hemiplegia after stroke admitted to the Fourth Hospital of Baotou, Inner Mongolia Autonomous Region from September 2020 to December 2022 were selected and they were divided into the experimental group and the traditional group according to random number table method, with 68 cases in each group. The traditional group was given routine rehabilitation training intervention measures, and the experimental group was given percutaneous acupoint electrical stimulation combined with acupuncture intervention on this basis. Both groups were treated for three months. National Institutes of health stroke scale (NIHSS) score, Basle index (BI) of daily living ability score, Fugl-Meyer assessment scale (FMA) score before and after intervention were compared between the two groups. The spatial parameters of walking in the two groups were observed before and after intervention, the changes of traditional Chinese medicine syndrome scores before and after intervention were compared between the two groups, and the quality of life scores of the two groups were compared after intervention, and the incidence of adverse reactions in the two groups were analyzed. Results After intervention, NIHSS scores of the two groups were lower than those before intervention, BI and FMA scores were higher than those before intervention; NIHSS scores of the experimental group were lower than those of the traditional group, BI and FMA scores were higher than those of the traditional group (P<0.05). After intervention, the walking speed of the two groups was faster than that before intervention, the step frequency was higher than that before intervention, and the step length of the affected side was longer than that before intervention; and the walking speed of the experimental group was faster than that of the traditional group, the step frequency was higher than that of the traditional group, and the step length of the affected side was longer than that of the traditional group (P<0.05). After intervention, the scores of hemiplegia, heminumbness, skew of tongue, and skew of speech in the two groups were lower than those before intervention; and the scores of hemiplegia, heminumbness, skew of tongue, and skew of speech in the experimental group were lower than those in the traditional group (P<0.05). The scores of social function, material life, physical function, and psychological function in experimental group were higher than those in traditional group (P<0.05). The incidence of pressure ulcer, pulmonary infection, urinary infection, muscle atrophy, movement abnormality, and shoulder and hand syndrome in experimental group were lower than those in traditional group (P<0.05). Conclusion Percutaneous acupoint electrical stimulation combined with acupuncture can improve the nerve function and motor function of patients with hemiplegia after stroke, improve their walking status, increase their traditional Chinese medicine syndrome score, improve their quality of life, and reduce adverse reactions, which is worthy of further clinical application.

Effect of acupoint electroacupuncture combined with intramuscular effect plaster in the treatment of shoulder hand syndrome after stroke

Objective To explore the effect of acupoint electroacupuncture combined with intramuscular effect plaster in the treatment of shoulder hand syndrome after stroke. Methods A total of 174 patients with shoulder hand syndrome after stroke admitted to Liaoning Jinqiu Hospital from February 2021 to September 2022 were selected and they were divided into control group A (58 cases), control group B (58 cases) and observation group (58 cases) according to random number table method. Control group A was treated with intramuscular effect plaster, control group B was treated with acupoint electroacupuncture, and observation group was treated with acupoint electroacupuncture combined with intramuscular effect plaster. All three groups were treated for three courses. Symptom scores, clinical efficacy, biomechanical indicators of shoulder and hand muscle groups and treatment safety were compared among three groups. Results After treatment, the scores of traditional Chinese medicine syndrome and shoulder hand syn drome assessment scale (SHSS) in three groups were lower than those before treatment, the scores of Fugl-Meyer assessment scale (FMA) in three groups were higher than those before treatment, and the scores of traditional Chinese medicine syndrome and SHSS in observation group were lower than those in control group A and control group B, FMA scores in observation group were higher than those in control group A and control group B, and the differences were statistically significant (P<0.05). Clinical efficacy of observation group was superior to that of control group A and control group B, the difference was statistically significant (P<0.05). After treatment, the peak torque of shoulder abductor muscle, anterior flexor muscle, internal rotator muscle, and external rotator muscle of three groups were higher than those before treatment, and those of observation group was higher than those of control group A and control group B, the differences were statistically significant (P<0.05). There was no statistically significant difference in the incidence rate of adverse reactions among observation group, control group A and control group B (P>0.05). Conclusion The acupoint electroacupuncture combined with intramuscular effect plaster in the treatment of shoulder hand syndrome after stroke has a significant effect, which can improve patient symptoms and shoulder muscle strength, improve limb motor function, and have good treatment safety.

Clinical effect of massage on infantile anorexia in children with spleen and stomach weakness type

Objective To analyze the clinical effect of massage on infantile anorexia with spleen and stomach weakness type. Methods A total of 104 children with anorexia admitted to Yongzhou Central Hospital of Hunan Province from February 2021 to October 2022 were selected and they were divided into experimental group (52 cases) and control group (52 cases) according to random number table method. Among them, the control group was given conventional treatment and Shenqu Xiaoshi Oral Liquid treatment, on the basis of the control group, the experimental group was given massage treatment. Both groups were treated continuously for two weeks. After two weeks of treatment, the clinical efficacy of two groups were compared, and the scores of traditional Chinese medicine symptoms, serological indicators, and the occurrence of adverse reactions during treatment were compared between two groups. Results The clinical efficacy of experimental group was better than that of control group, and the difference was statistically significant (P<0.05). After treatment, the scores of loss of appetite, decreased food intake, nausea, vomiting, abdominal discomfort, and irritability of two groups were all decreased compared with before treatment, and those of experimental group were lower than those of control group, and the differences were statistically significant (P<0.05). After treatment, the levels of serum gastrin, motilin, 5-hydroxytryptamine, and salivary amylase in two groups were all increased compared with before treatment, and those of experimental group were higher than those of control group, and the differences were statistically significant (P<0.05). No obvious adverse reactions were observed in both groups during treatment. Conclusion The effect of massage therapy on infantile anorexia with spleen and stomach weakness type is remarkable, which can further improve the clinical symptoms and gastrointestinal function of children with good safety.

Clinical effect of ultrasound-guided akupotomye sequential therapy in the treatment of scapulohumeral periarthritis

Objective To observe the clinical effect of ultrasound-guided akupotomye sequential therapy on scapulohumeral periarthritis. Methods Sixty patients with scapulohumeral periarthritis treated in Wangjing Hospital of Chinese Academy of Traditional Chinese Medicine from August 2021 to September 2022 were divided into treatment group and control group by random number table method, with 30 cases in each group. The treatment group was treated with ultrasonic guidance-hydraulic expansion-akupotomye release-shoulder joint loosening, while the control group was treated with ultrasonic guidance-pain point sealing-shoulder joint loosening. The course of treatment in both groups was three weeks. Visual analogue scale (VAS) and Constant-Murley shoulder function score were compared before treatment (T1), after the last treatment (T2) and two weeks after treatment (T3), the thickness of internodal sulci and subglenoid capsula at T1 and T3 were compared and the clinical efficacy of the two groups were compared. Results VAS and Constant-Murley shoulder joint function scores at different time points between the two groups were statistically significant (P<0.05). The VAS score at T2-T3 in the treatment group was lower than that in the control group, and the Constant-Murley shoulder joint function score was higher than that in the control group, the differences were statistically significant (P<0.05). The thickness of internodal sulci and subglenoid capsula decreased in T3 compared with T1, and the treatment group was lower than the control group, the differences were statistically significant (P<0.05). The clinical effect of the treatment group was better than that of the control group, and the difference was statistically significant (P<0.05). Conclusion Ultrasound-guided akupotomye sequential therapy has a good clinical effect on periarthritis of shoulder, which can relieve pain and improve mobility.

Effect of tuberculosis prevention and control based on “Internet + tuberculosis prevention and control” model

Objective To explore the the effect of tuberculosis prevention and control based on “Internet + tuberculosis prevention and control” model. Methods A total of 3 118 tuberculosis patients in Shache County, Kashgar Prefecture, Xinjiang Uygur Autonomous Region from April 2020 to April 2022 were collected and studied. They were divided into control group and study group, with 1 559 cases in each group. The control group was given the conventional tuberculosis prevention and control model intervention, while the study group was given the “Internet + tuberculosis prevention and control” model intervention, and both groups were intervened for six months. The clinical outcome, treatment compliance and psychological status of the two groups were compared. Results After investigation, 280 cases in the control group, and 225 cases in the study group were lost to follow-up. There was no statistically significant difference in the successful cure rate of patients between the two groups (P>0.05); there was no statistically significant difference in the interruption rate of treatment between the two groups (P>0.05). By setting the treatment outcome of patients lost to follow-up as unknown treatment outcome, the rate of unknown treatment outcome was lower in the study group than that in the control group (P<0.05). At six months of intervention, the compliance rate of the study group was better than that of the control group (P<0.05). At six months of the intervention, self-rating anxiety scale and self-rating depression scale scores of both groups decreased compared with those before treatment, and the study group was lower than that of the control group (P<0.05). Conclusion The application of “internet + tuberculosis prevention and control” model can improve the treatment compliance of tuberculosis patients, improve their negative emotions, reduce the rate of patients lost to follow up, and better meet the needs of tuberculosis prevention and control.

Application effect of “chain+strengths mode of case management” in rehabilitation of elderly patients with chronic obstructive pulmonary disease

Objective To explore application effect of “chain+strengths mode of case management” in rehabilitation of elderly patients with chronic obstructive pulmonary disease. Methods A total of 85 elderly patients with chronic obstructive pulmonary disease were selected from the Department of Respiratory and Critical Care Medicine of the First People’s Hospital of Yichang, Hubei Province from January to July 2021. Patients were divided into control group (42 cases) and experimental group (43 cases) according to the order of arrival of patients. The control group was given routine management, and the experimental group was administered “chain+strengths mode of case management” on the basis of the control group. Pulmonary function, quality of life, anxiety status were compared between the two groups before intervention, and 1 and 2 months after intervention by pulmonary function index, self-assessment test (CAT) questionnaire and 7-items generalized anxiety disorder scale (GAD-7) and pulmonary rehabilitation compliance. Results There were statistically significant differences pulmonary function,CAT, GAD-7 scores between groups, time points comparison and interaction (P<0.05). Further pair-wise comparison showed that there were statistically significant differences in lung function, CAT, and GAD-7 scores between the two groups at each time points (P<0.05), and after two months intervention, forced expiratory volume in one second, forced expiratory volume in one second/ forced vital capacity of the experimental group were higher than thlse of the control group, and the CAT and GAD-7 scores were lower than those of the control group (P<0.05). The compliance of pulmonary rehabilitation in experimental group was better than that in control group(P<0.05). Conclusion The application of “chain + strengths mode of case management” in pulmonary rehabilitation management can improve pulmonary function indicators, quality of life and anxiety in elderly patients with chronic obstructive pulmonary disease, and improve the patients’ pulmonary rehabilitation compliance.

Effect of disease information support model on prenatal coping ability and pregnancy outcome of patients with placenta previa

Objective To explore the effects of disease information support model on prenatal coping ability and postpartum pregnancy outcome of placenta previa patients. Methods A total of 90 patients with placenta previa admitted to Huaian First People’s Hospital, Jiangsu Province from May 2020 to April 2022 were selected as the study objects, and they were divided into observation group and control group by random number table method, with 45 cases in each group. The control group implemented the conventional health education model, and the observation group implemented the disease information support model based on the control group. The coping ability and anxiety state before intervention and eight weeks after intervention were compared between the two groups, and the pregnancy outcomes in the two groups were recorded. Results After eight weeks of intervention, the scores of prenatal coping ability in both groups were higher than those before intervention, and those of observation group were higher than those of control group, the differences were statistically significant (P<0.05). After eight weeks of intervention, the self-rating anxiety scale scores of both groups were lower than those before intervention, and those of observation group were lower than those of control group, the differences were statistically significant (P<0.05). The total incidence of postpartum hemorrhage, premature infants, and neonatal asphyxia in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). Conclusion Disease information support can improve the stress coping ability of placenta previa patients, relieve psychological anxiety, and reduce the occurrence of maternal and infant adverse events.

Application of family follow-up care under the guidance of IKAP model in pregnant women with heart disease

Objective To analyze the application of family follow-up care under the guidance of IKAP model in pregnant women with heart disease. Methods A total of 78 pregnant patients with heart disease in the Department of Obstetrics, Rugao People’s Hospital, Jiangsu Province from March 2019 to March 2021 were selected, they were divided into control group and observation group by random number table method, with 39 cases in each group. The control group received routine family extended care intervention, while the observation group received family follow-up care under the guidance of IKAP model. Hamilton anxiety scale (HAMA), Hamilton depression scale (HAMD), and 36-item short form health survey (SF-36) were used to evaluate the incidence of negative emotions, complications and quality of life. Results After intervention, HAMA and HAMD scores in both groups were lower than those before intervention, and the observation group was lower than that of the control group, and the differences were statistically significant (P<0.05). The total incidence of complications in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). The scores of overall health, mental health, emotional function, and physiological function of the two groups were higher than those before intervention, and the observation group was higher than that of the control group, the differencel were statistically significant (P<0.05). Conclusion Family follow-up care under the guidance of IKAP model is applied to patients with pregnancy complicated with heart disease, so as to ease bad psychological emotions, reduce the occurrence of complications during pregnancy and delivery, and improve their quality of life.

Application effect of baduanjin in stage Ⅰ cardiac rehabilitation of patients with acute myocardial infarction after percutaneous coronary intervention

Objective To investigate the effect of baduanjin on stage Ⅰ cardiac rehabilitation in patients with acute myocardial infarction after percutaneous coronary intervention (PCI). Methods A total of 90 patients with acute myocardial infarction after PCI admitted to Suzhou Science and Technology City Hospital, Nanjing Medical University from June 2020 to June 2021 were selected and divided into experimental group and control group by random number table method, with 45 cases in each group. The control group was given routine nursing intervention and cardiac rehabilitation training, and the observation group was given routine nursing intervention and baduanjin rehabilitation training. After three months of training, serum biochemical indexes, imaging indexes, cardiopulmonary exercise test, and incidence of major cardiac events were compared between the two groups. Results After intervention, cardiac troponin I (cTnI), brain natriuretic peptide (BNP), and left ventricular end-diastolic diameter (LVED) in both groups were lower than those before intervention, and left ventricular ejection fraction (LVEF) was higher than that before intervention, and cTnI, BNP, LVED in experimental group were lower than those in control group, and LVEF in experimental groups was higher than that in control group, the differences were statistically significant (P<0.05). After intervention, anaerobic threshold, maximal oxygen uptake, and energy consumption level in two groups were higher than those before intervention, and those of experimental group were higher than those of control group, the differences were statistically significant (P<0.05). There was no significant difference in the incidence of major cardiac events in three months between the two groups (P>0.05). Conclusion Baduanjin can improve the cardiac function of patients with stage Ⅰ cardiac rehabilitation after PCI, and its safety is good.

Influence of intensive nursing under the concept of accelerated rehabilitation surgery on the prognosis effect of patients with femoral head necrosis undergoing total hip replacement

Objective To explore the influence of intensive nursing under the concept of accelerated rehabilitation surgery in prognosis effect of patients with femoral head necrosis undergoing total hip replacement. Methods A total of 148 patients with femoral head necrosis who underwent total hip replacement surgery in the 904th Hospital of the Chinese People’s Liberation Army Joint Logistic Support Force from April 2020 to May 2022 were selected and divided into reference group and intervention group by random number table method. The reference group (74 cases) received routine rehabilitation nursing intervention measures. The intervention group (74 cases) implemented the intensive nursing intervention measures under the concept of accelerated rehabilitation surgery. After one day of admission and 14 days of intervention, exercise fear (Chinese version of the kinetic phobia assessment table), joint injury (osteoarthritis index), and hip motion were analyzed in the two groups. Results After 14 days of intervention, the scores of Chinese version of the kinetic phobia assessment table and osteoarthritis index in both groups were lower than those at one day of admission, and those of intervention group were lower than those of reference group, the differences were statistically significant (P<0.05). After 14 days of intervention, the degree of joint extension, adduction, flexion, and abduction in the intervention group were higher than those in the reference group, and the differences were statistically significant (P<0.05). Conclusion Intensive nursing intervention under the concept of accelerated rehabilitation surgery can alleviate the fear of movement in patients undergoing total hip replacement with femoral head necrosis, improve the condition of joint injury, and increase the range of hip joint movement.

Research on the status and equity of traditional Chinese medicine medical resource allocation in Tianjin

Objective To analyze the development status and equity of traditional Chinese medicine medical resources allocation in Tianjin from 2011 to 2020, and to provide suggestions for the development of traditional Chinese medicine health undertakings in Tianjin. Methods Data on the allocation of traditional Chinese medicine hospitals, hospital beds, and health technicians in Tianjin during 2011—2020 were obtained from Tianjin Health Statistics. Descriptive analysis method was used to analyze the overall situation of traditional Chinese medicine medical resource allocation in Tianjin, and Gini coefficient and difference index were used to evaluate the fairness of traditional Chinese medicine medical resource allocation in Tianjin. Results During 2011—2020, the total amount of traditional Chinese medicine medical resources and the possession of each thousand population in Tianjin had increased greatly, and the three types of resources had increased by more than 50%. The Gini coefficients of population, geography and gross domestic product (GDP) were all >0.4, except for some indicators in some years. The difference index calculated by geography was >0.5, the difference index calculated by population was <0.5, and the difference index calculated by GDP was 0.4-0.6. The Gini coefficient and the difference index of beds by population decreased from 0.40 to 0.39, but the Gini coefficient and difference index of beds by population and geographical allocation increased, and the Gini coefficient and difference index of institutional allocation increased by more than 30%, while the equity of resource allocation decreased. The Gini coefficient and difference index of GDP allocation decreased by 3%-12%, and the fairness of resource allocation increased. Conclusion We should change the management mode of traditional Chinese medicine medical resources, strengthen the allocation of resources in remote and rural areas, control the expansion of traditional Chinese medicine hospitals in economically developed areas, and achieve the expansion of high-quality traditional Chinese medicine medical resources and a balanced regional layout.

Analysis of the changing trend and influencing factors of resource allocation efficiency of maternal and child health care institutions in China based on DEA and ARIMA models

Objective To explore the changing trend and key influencing factors of health resource allocation efficiency in maternal and child health care institutions in China, and to put forward policy suggestions for rational allocation of health resources. Methods The index data of basic configuration (number of institutions, number of beds, and health technicians), service quantity (total number of visits, number of inpatients) and service efficiency (number of bed turnover, bed working days, and bed utilization rate) of maternal and child health institutions were selected from China Statistical Yearbook and China Health Statistical Yearbook from January 2010 to December 2020. The autoregressive integrated moving average model was used to predict the change trend of each index from 2021 to 2025, and the data envelopment analysis (DEA) model was used to comprehensively evaluate the input-output efficiency from 2010 to 2025, and the key influencing factors were found out. Results It was predicted that 2022—2025 would be a non-DEA effective year. From 2010 to 2025, the average values of input-output comprehensive technical efficiency change index, pure technical efficiency, and scale efficiency were one, and the average values of technological progress index and total factor productivity index were 0.971. The main influencing factor was technological progress index. In 2020, the lowest comprehensive benefit was 0.872, and the lowest total factor productivity index was 0.825. Conclusion The basic allocation input and service quantity output of maternal and child health care institutions in China are relatively high, and the service efficiency has not been significantly improved. The technological progress index is the main influencing factor that affects the output efficiency and improves the total factor productivity index, which is paid special attention during the “14th Five-Year Plan” period.
