DOI: 10.20047/j.issn1673-7210.2023.20.01
Objective To explore the apoptosis-related long non-coding RNA(lncRNA)-mRNA network in ischemic stroke (IS) and predict the hub genes. Methods A total of 10 patients with IS admitted to the First Affiliated Hospital of Guangxi University of Chinese Medicine (“our hospital” for short) from January 2016 to June 2017, and 10 healthy controls from community health examination center of our hospital during the same period were selected as the research objects. Differential analysis of microarray expression profiles was performed, weighted gene co-expression network analysis was used to screen out genes in the target module, and the intersection with apoptotic genes was taken. Top 10 differentially mRNAs (DEMs) were selected using the STRING website and Cytoscape software, and lncRNA-mRNA co-expression network was constructed between DEMs and differentially lncRNAs (DELs). Finally, hub genes were obtained. Results A total of 3 930 DEMs were obtained, and 553 apoptosis-related IS genes were obtained after intersection of module genes and apoptosis genes. The lncRNA-mRNA co-expression network consisted of 9 DEMs and 277 DELs. Finally, 7 hub genes G015366, G068382, PIK3CB, BPTF, NR3C1, Akt2, and INS were obtained. Conclusion G015366, G068382, PIK3CB, BPTF, NR3C1, Akt2, and INS are related to the occurrence and development of IS, and may become diagnostic markers of IS.

DOI: 10.20047/j.issn1673-7210.2023.20.08
In recent years, the incidence of chronic kidney disease has been increasing, and the onset of chronic kidney disease is becoming younger, which has caused serious harm to human life and health. Microinflammatory state is one of the common pathological changes of chronic kidney disease and an important factor leading to the occurrence and development of chronic kidney disease. Studies have shown that autophagy is involved in the occurrence and development of chronic kidney disease and promoting autophagy can improve the microinflammatory state of chronic kidney disease. Based on the new target of autophagy treatment, the application of traditional Chinese medicine to promote autophagy has obvious advantages in improving the microinflammatory state of chronic kidney disease. The monomeric components of Radix et Rhizoma Rhei in Chinese herbal medicine and its composition of drug pairs and compounds can play the role of anti-inflammation, anti-oxidative stress, anti-apoptosis, and anti-fibrosis by promoting autophagy, which is of great significance in delaying the progression of chronic kidney disease, and is expected to become one of the effective treatment methods for chronic kidney disease and its complications.

DOI: 10.20047/j.issn1673-7210.2023.20.09
Idiopathic pulmonary fibrosis (IPF) is a refractory pulmonary disease. With the progress of the disease, the pulmonary function of the patients decreases progressively, which seriously affects the quality of life. Drug treatment has delayed the disease progression to a certain extent, and won more conditions and time windows for pulmonary rehabilitation treatment. Traditional Chinese and western medicine in pulmonary rehabilitation strategies such as exercise training, respiratory training, oxygen therapy, disease self-management, external treatment of traditional Chinese medicine and traditional Chinese medicine functional exercise can improve the exercise endurance, quality of life and reduce the risk after lung transplantation of IPF patients. However, there are still many problems to be further studied in IPF pulmonary rehabilitation. The article reviews the latest research progress of pulmonary rehabilitation in patients with IPF around the training methods and effect evaluation of pulmonary rehabilitation.

52 Research progress of the osteocyte-related biomarkers in predicting osteoporosis and fragility fractures
LI Shiwei1 WANG Boling2 SHI Cheng2 SUN Zhongyang3
DOI: 10.20047/j.issn1673-7210.2023.20.10
The traditional bone turnover markers derived from osteoblasts and osteoclasts are useful in the diagnosis and treatment of osteoporosis. However, the usefulness of bone turnover markers in predicting osteoporosis and fragility fractures remains elusive. As the most abundant cell in bone tissue, osteocytes play a central role in regulating bone turnover. Recent research shows that bone turnover markers secreted by osteocytes, such as sclerostin, DKK1, and fibroblast growth factor 23 could be a new type of bone turnover markers for osteoporosis and fragility fracture prediction. This paper will briefly summarize the new research progress of the osteocyte related biomarkers with clinical application prospects in the prediction of osteoporosis and fragility fracture, in order to provide theoretical basis for the diagnosis and evaluation of osteoporosis and fragility fractures.

DOI: 10.20047/j.issn1673-7210.2023.20.12
Non-alcoholic fatty liver disease is one of the common chronic liver diseases in the world, and its incidence is on the rise in China. At present, its pathogenesis is not clear, but it is widely involved in liver glucose, lipid metabolism, insulin resistance, oxidative stress, and so on. Recent studies have found that skeletal muscle can affect non-alcoholic fatty liver disease by releasing muscle factors such as irisin, fibroblast growth factor-21, insulin-like growth factor-1, interleukin-6, tumor necrosis factor-α, and vascular endothelial growth factor B. In this paper, the correlation between some common muscle factors and non-alcoholic fatty liver disease and the possible mechanism of action in this disease are reviewed in order to provide a new perspective and thinking for the prevention and treatment of non-alcoholic liver disease.

65 Research progress on perioperative cough in patients undergoing lung surgery
YAN Tingzhi1 WU Di1 LIU Hongyu2 CHEN Jun1
DOI: 10.20047/j.issn1673-7210.2023.20.13
Cough is one of the common symptoms and complications of patients undergoing lung surgery, which often has a great impact on the recovery process and quality of life of patients after surgery. Early identification and management of the risk factors of perioperative cough in patients undergoing lung surgery are the basis of effective intervention strategies. At present, many clinical and basic studies at home and abroad have tried to put forward a systematic management plan for patients with cough during perioperative period, but the results are not satisfactory. Through searching the relevant literature reports of cough symptoms during the perioperative period of lung surgery in recent years, this article found that women, right lung cancer, difficult airway, acute cough, history of chronic obstructive pulmonary disease, mediastinal lymph node dissection, length of bronchial stump, anesthesia, postoperative pleural effusion, and bronchial kink are all related to the perioperative cough of lung surgery, and the conversion receptor potential cation channel subfamily V member 1 pathway may be its potential molecular mechanism. The management of perioperative cough during lung surgery includes drug treatment, lymph node residual cavity filling, intraoperative nerve monitoring, bronchial prosthesis placement, acupuncture, postoperative respiratory exercise, etc. These findings can provide reference for clinical early identification of high-risk patients with cough during the perioperative period of lung surgery, as well as the prevention and management of cough symptoms, and provide direction for further research.

DOI: 10.20047/j.issn1673-7210.2023.20.20
Objective To identify the β-globin gene sequence of suspected cases of β-thalassemia (“β-thalassemia” for short”), the rare mutation site identified and the clinical phenotype were analyzed. Methods From January 2020 to March 2022, 20 suspected β-thalassemia carriers with hematologic phenotype but no related β-thalassemia gene test results were negative in Guizhou Provincial People’s Hospital. The full length of β-globin gene was amplified by PCR, The amplification products were sequenced bidirectionally by Sanger sequencing technique. The sequencing results were compared with the β-globin gene reference sequence to find out whether there were rare mutations. Results Among 20 Carriers with suspected β-thalassaemia, 15 were found to carry rare β-globin gene mutations, with a total of six mutation types: CD53(-T), CD5(-CT), CD8(-AA), -88(C>A), Cap+22(G>A) and -50(G>A). Except for one case of -50(G>A) carrier, the other 14 cases showed the clinical phenotype of β-thalassaemia minor. Conclusion Rare β-globin gene analysis in suspected carriers whose phenotypes were not consistent with their genotypes plays an important role in guiding β-thalassemia genetic counseling, prenatal diagnosis and reducing the birth of children with moderate and severe β-thalassaemia. The results of this study broaden the mutation spectrum of β-globin gene in Chinese population and Guizhou region.

DOI: 10.20047/j.issn1673-7210.2023.20.21
Objective To analyze the correlation of carotid plaques with fibrinogen (FIB), cystatin C (Cys-C), and oxidized low-density lipoprotein (ox-LDL) in patients with acute cerebral infarction. Methods The clinical data of 97 patients with acute cerebral infarction admitted to Suqian Zhongwu Hospital from May 2020 to April 2021 were retrospectively analyzed. According to the presence or absence of carotid plaque formation, they were divided into the non-plaque group (46 cases) and the plaque group (51 cases). The clinical data of the two groups were compared, and the risk factors of carotid plaque formation in patients with acute cerebral infarction were analyzed by multivariate logistic regression  analysis. The receiver operating characteristic (ROC) curve was made, and the area under the curve (AUC) was used to analyze the predictive value of serum FIB, Cys-C, ox-LDL and their combination on carotid plaque formation in patients with acute cerebral infarction. Results The proportion of hyperlipidemia and hypertension, and the levels of total cholesterol, serum FIB, Cys-C, and ox-LDL at admission in the plaque group were higher than those in the non-plaque group (P<0.05). Multivariate logistic regression analysis showed that hyperlipidemia and high serum FIB, Cys-C, and ox-LDL levels on admission were risk factors for carotid plaque formation in patients with acute cerebral infarction (OR>1,P<0.05). ROC results showed that, the AUC value of combined prediction of carotid plaque formation in patients with acute cerebral infarction was higher than that of single prediction (P<0.05). Conclusion Serum levels of FIB, Cys-C and ox-LDL have important value in predicting carotid plaque formation in patients with acute cerebral infarction, and the combination of the three has a higher predictive value.

DOI: 10.20047/j.issn1673-7210.2023.20.23
Objective To analyze the risk factors for early neurological deterioration (END) in patients with isolated subcortical infarction. Methods A total of 422 patients who were hospitalized in the Department of Neurology of Cangzhou Central Hospital from January 2021 to March 2022 and diagnosed as isolated subcortical infarction by MRI were selected as the research objects. National Institutes of Health stroke scale (NIHSS) score was used to assess the degree of neurological impairment of all patients on the day of admission and within three days after admission. If the NIHSS score increased ≥2 points within 72 hours after admission, it was considered that END had occurred. Then, patients were divided into the END group (81 cases) and the non-END group (341 cases) according to whether END occurred. Baseline data and risk factors of the two groups were compared, and the independent risk factors for predicting END were screened out. Results There were significant differences in age, male ratio, stroke history, lesion diameter, and carrier artery stenosis between the two groups (P<0.05). Logistic regression analysis showed that male (OR=3.084, 95%CI:1.500-6.340, P=0.002), stroke history (OR=2.391, 95%CI:1.158-4.937, P=0.018), lesion diameter >2 cm (OR=3.038, 95%CI:1.434-6.436, P=0.004) and carrier artery stenosis (OR=14.157, 95%CI: 7.602-26.363, P<0.001) were independent risk factors for END in patients with acute isolated subcortical infarction. Conclusion Isolated subcortical infarction patients with male, stroke history, lesion diameter>2.0 cm, and carrier artery stenosis are more likely to develop END, which should arouse clinical attention.

The wound healing of patients one week after tooth extraction was counted, and they were divided into poor healing group and good healing group according to the wound healing condition. The clinical data and the difference of systolic blood pressure and diastolic blood pressure before and after tooth extraction between two groups were compared; the influencing factors of wound healing effect in elderly hypertension patients were analyzed; the predictive value of blood pressure level before and after tooth extraction for wound healing was evaluated. Results There were 94 cases of good wound healing and 21 cases of bad wound healing in 115 elderly patients with hypertension. The level of serum interleukin-6 (IL-6) and the difference of systolic blood pressure and diastolic blood pressure before and after extraction in poor healing group were higher than those in good healing group (P<0.05). IL-6, the difference of systolic blood pressure and diastolic blood pressure before and after tooth extraction were risk factors for poor wound healing in elderly patients with hypertension (OR=3.080, 2.954, 3.350, P<0.05). The area under the curve of combined systolic blood pressure and diastolic blood pressure before and after tooth extraction was higher than that of each single prediction (P<0.05). Conclusion The blood pressure level before and after tooth extraction is related to the wound healing effect in elderly patients with hypertension, and the combined measurement of the difference of systolic blood pressure and diastolic blood pressure before and after tooth extraction can predict the wound healing effect in elderly patients with hypertension.

Insomnia, is due to emotion, diet, internal injury, and many other causes, often can not get normal sleep as a kind of disease syndrome, of which phlegm-fire disturbing heart type is quite common in clinical. Huang Yuanyu, a doctor in the Qing Dynasty, held the idea of “cycle flow of one qi”, from the point of view of qi of spleen and stomach as the basis hub to transfer the other four of the five elements, names wood, fire, gold, and water, to explore the etiology, pathogenesis, syndrome, and treatment of the disease. He believes that the basic pathogenesis of insomnia with phlegm-fire disturbing heart type is the decay of spleen and stomach, phlegm-turbidity disturbing clear orifices, and ministerial fire growth. Based on this, a distinctive treatment method is formed, that is banking up earth to control water, diving ministerial fire and eliminating phlegm-turbidity. In the course of treatment, the application of Huangya Decoction and Jinding Decoction is mostly based on addition and subtraction, in order to the spleen and stomach and make the movement of qi rise and fall harmoniously, to achieve the purpose of phlegm fire not produce and sleep orderly.

DOI: 10.20047/j.issn1673-7210.2023.20.43
Objective To conduct an econometric analysis of the current research progress at the intersection of media and psychology. Methods All literatures in the cross-field of media and psychology from January 2018 to June 2022 were collected on Scopus. After screening, VOSviewer was used to conduct a quantitative analysis in terms of connection strength, time, word agglomeration, and other dimensions from the perspectives of author and keywords. Results A total of 3 233 literatures were selected, and it was found that the research subject in February 2022 was Griffiths MD. The scholars group with the core of Li was transferred to the Chinese scholars group with the core of Li. The overall number of connections among Chinese scholars was large, but the individual connection strength was weak. The total number of published papers was large, but the number of citations per paper was low. 40% of the authors’ keywords focused on the public mental health state influenced by the media; there were word agglomerates named after gender and age stage. Information epidemiology was widely mentioned. Conclusion The research foundation of domestic academic circles in this field is large, but the influence needs to be improved. Depression and anxiety caused by the influence of media and their mental health categories are the mainstream research directions at present. Dichotomy of gender and age are the main research method of population classification. Information epidemiology has the potential to develop further as a new discipline.
