4 Effect of Ganlu Xiaodu Micropills and its decoction parts on the expression of inflammatory factors in NR8383 cells treated with Mycoplasma pneumoniae

Objective To study the effect of Ganlu Xiaodu Micropills and its decoction on the expression of inflammatory factors in rat alveolar macrophages (NR8383) infected with Mycoplasma pneumoniae (MP). Methods Thirty-five female Wistar rats of SPF grade aged eight weeks, weighing (200±10) g, were selected. According to the random number table method, they were divided into blank serum group (normal saline 2 ml/d), Ganlu Xiaodu Micropills serum group (33 g/[kg·d]), Azithromycin serum group (0.063 mg/[kg·d]), decoction part 1 serum group (12.3 g/[kg·d]), decoction part 2 serum group (20.8 g/[kg·d]), with seven rats in each group. Each group of doses was given once a day for one week to prepare drug- containing serum. NR8383 cells were treated with blank serum and drug-containing serum, respectively, CCK-8 assay was used to detect and compare the difference of viability between NR8383 cells treated with different serum and untreated cells. NR8383 cells were divided into control group, model group, Ganlu Xiaodu Micropills group, Azithromycin group, and decoction part 1, 2 groups. Except for the control group, the other groups were treated with 106-107 CCU/ml MP bacterial solution for four hours to establish MP infection model, and then the corresponding drug-containing serum was added and incubated for 24 hours. The cells and supernatant were collected, and qPCR and Western blot were used to detect the mRNA and protein expression of Toll-like receptor 4 (TLR4), MyD88, and nuclear factor κB (NF-κB) in each group. The expression levels of tumor necrosis factor-α (TNF-α), interleukin (IL)-1β, and IL-10 in each group were detected by enzyme-linked immunosorbent assay. Results The survival rate of cells treated with each serum were lower than those of cells without treatment (P<0.05). There was no significant difference in the survival rate of cells treated with different serum (P>0.05). The expression levels of TLR4, MyD88, and NF-κB-p50 mRNA and protein in the model group were higher than those in the control group, and the expression levels of TLR4, MyD88, and NF-κB-p50 mRNA and protein in the Ganlu Xiaodu Micropills group and the decoction part 2 group were lower than those in the model group, the expression of MyD88 mRNA and TLR4 protein in the decoction part were lower than those in the model group (P<0.05). The mRNA expression of TLR4 and MyD88, and the protein levels of TLR4, MyD88, NF-κB-p50 in Ganlu Xiaodu Micropills group were lower than those in decoction part 1 group, and the mRNA expression of NF-κB-p50 and the protein level of TLR4 were higher than those in decoction part 2 group (P<0.05). The levels of TNF-α and IL-1β in Ganlu Xiaodu Micropills group and decoction part 2 group were lower than those in model group, the level of IL-1β in decoction part 1 group was lower than that in model group, and the level of IL-10 in decoction part 2 group was lower than that in model group (P<0.05). The levels of TNF-α, IL-1β, and IL-10 in Ganlu Xiaodu Micropills group were lower than those in decoction part 1 group, and the levels of TNF-α and IL-1β were higher than those in decoction part 2 group (P<0.05). Conclusion MP infection can promote the expression of TNF-α, IL-1β, and IL-10 in NR8383 cells. Ganlu Xiaodu Micropills and its decoction containing serum can reduce the inflammatory response by inhibiting the expression of TNF-α and IL-1β, and the mechanism may be related to the TLR4/MyD88/NF-κB pathway.

10 Determination of residual ethanol and acetone in refined polysaccharide from pneumococcal serotype 6B by headspace gas chromatography

Objective To establish a headspace gas chromatography method for the determination of residual ethanol and acetone in the refined polysaccharide from pneumococcal serotype 6B. Methods The residual amounts of ethanol and acetone in the refined polysaccharide from pneumococcal serotype 6B by Beijing Minhai Biotechnology Co., Ltd. were determined by headspace gas chromatography with water as solvent. The chromatographic conditions were as follows: DB-624 capillary column (30 m × 0.25 mm, 1.80 μm); the flow rate of carrier gas chromatographic column was 3.00 ml/min, and the split ratio was 10∶1. The initial column temperature was maintained at 40℃ for 2 min, then raised to 80℃ at a rate of 5℃ per minute for 4 min; and raised to 200℃ at a rate of 20℃ per minute for 2 min; The detector temperature was 240℃. The temperature of thermostatic furnace was 80℃ and the equilibrium time was 30 min. The system applicability, specificity, detection limit, quantitative limit, linear range, accuracy, repeatability, and reproducibility of the method were verified. Results The theoretical plate number of ethanol and acetone was≥5 000, and the resolution was ≥1.5. Phenol had no interference to the residue of ethanol and acetone. The limit of detection of ethanol was 0.199 μg/ml, and the limit of quantitation was 0.596 μg/ml. The detection limit of acetone was 0.051 μg/ml, and the limit of quantitation was 0.152 μg/ml. The linear equation of ethanol was Y=2 276.471 86X-265.405 07, r=0.999 99, and the linear range was 0.090 - 30.000 μg/ml. The linear equation of acetone was Y=7 790.898 42X+4 216.806 23, r= 0.998 13, and the linear range was 0.120-40.000 μg/ml. The accuracy recovery rate of from pneu- mococcal serotype 6B was 86.20 %-100.66 % for ethanol and 82.89 %-91.42 % for acetone. The repeatability RSD was 2.3 % for ethanol and 5.5 % for acetone. The RSD of intermediate precision was 7.2% for ethanol and 4.2% for acetone. The residual range of ethanol in 9 batches of samples was not detected to 6.184 μg/ml, and the residual range of acetone was not detected to 10.475 μg/ml. Conclusion The method of headspace gas chromatography for the determination of ethanol and acetone residues in refined polysaccharide from  pheumococcal serotype 6B has good system suitability, accuracy, precision and linearity. The method is rapid, simple and reliable. It can be used for the determination of ethanol and acetone residues in refined polysaccharide from pheumococcal serotype 6B.

15 Prediction of T/B cell epitopes specific to middle east respiratory syndrome coronavirus by immunoinformatics method

Objective To predict the T/B cell epitopes against middle east respiratory syndrome coronavirus (MERS-CoV) by immunoinformatics methods. Methods After obtaining S protein sequence from NCBI, MEGA11 was used to compare multiple sequences and construct phylogenetic tree. The physicochemical properties of S protein were analyzed by Expasy Protparam and its secondary structure was predicted by SOPMA. Subsequently, S protein was modeled and verified. Killer T cell (CTL) epitopes were predicted by NetCTL-1.2, NetMHC pan EL 4.1, and IEDB, while helper T cell (HTL) epitopes were predicted by NetMHCⅡpan-4.0, IFNepitope, and IL-4pred. ABCpred and BepiPred-2.0 predict linear B cell epitopes (LBL), and the ElliPro tool predicts conformational B cell epitopes (CBL). Finally, the antigenicity, sensitization, and toxicity of the predicted linear epitopes were predicted. Results The S protein sequence was highly conserved, and 100 MERS-CoV S proteins from different countries could fit into the same phylogenetic clade. The results of physicochemical analysis showed that the total mean value of hydrophilicity of S protein was -0.078, and the half-life of S protein in mammalian reticulocytes was about 30 h. The model verification results showed that the model of S protein was reasonable. Two CTL epitopes, two HTL epitopes, and 15 LBL epitopes with antigenicity, non-allergenicity and non-toxicity were predicted from S protein. The ElliPro tool predicted five CBL epitopes. Conclusion The S protein of MERS-CoV is a hydrophilic stable protein. T/B cell antigen epitopes can be predicted by combining various bioinformatics methods, which has important reference significance for the development of polypeptide vaccine against MERS-CoV.

20 Study of ultrasound guided needle knife release in of clinical anatomical extremity diseases

Objective To investigate the safety and accuracy of ultrasound guided needle knife release in extremity disease. Methods Twenty human specimens fixed with formalin were selected from the Department of Human Anatomy, School of Basic Medical Sciences, Peking University, the study was conducted from March 2019 to June 2022. Specimens were divided into traditional operation group and ultrasonic guided group, with ten specimens in each group, and the needle knife release operation was carried out under the same approach. The traditional operation group performed the needle knife release operation under invisible conditions, while the ultrasonic guided group performed the operation under the guidance of the high frequency probe. After the operation of needle loosening, the needle was kept, dissected layer by layer, and the needle path was observed, the success rate, accuracy and injury rate of conventional operation and ultrasound guided operation were compared in four diseases of radial stylostenosis tenosynovitis, flexor constriction tenosynovitis, carpal tunnel syndrome, and malleolar tunnel syndrome. Results The success rate of operation on carpal tunnel syndrome in ultrasound guided group was higher than that in traditional operation group, and the difference was statistically significant (P<0.05). The operation release efficiency of flexor stenosis tenosynovitis, carpal tunnel syndrome, and ankle tunnel syndrome in ultrasound guided group was higher than that in traditional operation group, and the differences were statistically significant (P<0.05). The operation injury rate of ultrasound guided group was lower than that of traditional operation group, and the difference was statistically significant (P<0.05). Conclusion Ultrasound guided needle knife release technique has high safety and accuracy in the operation of terminal extremity diseases, which is worthy of clinical promotion.

25 Study on the mechanism of DIO1 inhibiting invasion and metastasis in laryngeal squamous cell carcinoma

Objective To study the regulatory effect of deiodinase iodothyronine type Ⅰ (DIO1) on invasion and metastasis in laryngeal squamous cell carcinoma (LSCC). Methods The TU686 cells were divided into control group (normal TU686 cells) and DIO1 overexpression group (transfection of DIO1), and DIO1-1 μg group and DIO1-2 μg group were further divided according to overexpression level. DIO1 gene expression was detected by RT-qPCR, cell proliferation was detected by CCK-8 assay, cell apoptosis was detected by PI/DAPI staining, cell migration and invasion ability were detected by scratch assay and Transwell assay, the enrichment of related gene sets was analyzed by gene probe enrichment analysis, and the protein expression levels of DIO1, E-cadherin, N-cadherin, Vimentin, p-S6K1, p-4EBP1, and p-Akt were detected by Western blot. Results DIO1 mRNA and protein level of DIO1 overexpression group were higher than those of control group (P<0.01). At 24, 48, 72 h, optical density values of DIO1 overexpression group were lower than those of control group (P<0.01). The positive percentage of PI staining in DIO1 overexpression group was higher than that in control group (P<0.01). The relative migration distance of DIO1 overexpression group was shorter than control group (P<0.01).Migration and invasion ability of DIO1 overexpression group was lower than control group (P<0.01). DIO1 expression was negatively correlated with the expression of epithelial-mesenchymal transition and mTORC1 signaling pathway genes(NES<0). E-cadherinin DIO1-1 μg group and DIO1-2 μg group were higher than those in control group, while N-cadherin and Vimentin, p-S6K1, and p-4EBP1 were lower than those in control group (P<0.01). There were no significant differences in p-Akt between control group and DIO1-1 μg group, DIO1-2 μg group (P>0.05). Conclusion DIO1 can be a tumor suppressor gene and play a key role in the invasion and metastasis of LSCC.

30 Optimization of extraction process and antioxidant activity of total phenol from Rhodiola crenulata(HooK. f. et.Thoms)H. Ohba

Objective To optimize the extraction process of total phenol from Rhodiola crenulata(HooK. f. et.Thoms)H. Ohba by response surface method, and to analyze the content of total phenol and antioxidant activity of Rhodiola crenulata(HooK. f. et.Thoms)H. Ohba and their correlation, so as to provide scientific basis for resource exploitation and appropriate harvesting time of Rhodiola crenulata(HooK. f. et.Thoms)H. Ohba. Methods On the basis of single factor test, Box-Behnken response surface method was used to optimize the extraction process of total phenol from Rhodiola crenulata(HooK. f. et.Thoms)H. Ohba. The effects of Rhodiola crenulata(HooK. f. et.Thoms)H. Ohba extract on the scavenging ability of DPPH· and ABTS+· were determined. Results The optimum extraction conditions of Rhodiola crenulata(HooK. f. et.Thoms)H. Ohba were as follows: ethanol concentration of 50%, extraction temperature of 50℃, and extraction time of 40 min. Under these conditions, the extraction rate could reach 12.22 mg/g. The total phenol content and antioxidant activity of wild samples were significantly higher than those of artificial cultivars, and the total phenol content of artificial cultivars was the highest on September 20. The DPPH· scavenging ability of Rhodiola crenulata(HooK. f. et.Thoms)H. Ohba extract was stronger than ABTS+· scavenging ability. The total phenol content was negatively correlated with median inhibitory concentration(IC50) values of DPPH· and ABTS+·(r<0, P<0.01). Conclusion The theoretical values of the response surface optimization method are consistent with the measured values, which can be used to extract and predict the total phenol of Rhodiola crenulata(HooK. f. et.Thoms)H. Ohba. Rhodiola crenulata(HooK. f. et.Thoms)H. Ohba have strong antioxidant activity in vitro and is positively correlated with total phenol content. Oral cavity is a complex multi-microbial environment. A wide variety of microorganisms form oral flora, which constitutes the oral microecosystem together with the oral environment. Oral flora can be affected by many factors, such as gender, age, race, and living habits, etc., and plays an important role in maintaining oral and general health, and the occurrence and development of diseases. The development of high-throughput sequencing technology and bio-information technology has laid a foundation for the comprehensive study of oral flora. Accordingly, the structure and function of oral flora and its relationship with human health and disease are increasingly clear. This article reviews the research progress of oral flora, its association with oral diseases and systemic diseases, and looks forward to the future research direction.

39 Research progress on the mechanism of traditional Chinese medicine polysaccharide in treating depression

Traditional Chinese medicine polysaccharide has anti-inflammatory, antioxidant and immunomodulatory activities, and its advantages lie in low toxicity and side effects, high safety, and diverse functions. Polysaccharide can inhibit the progression of depression through inflammatory response pathway, neurotransmitter pathway, HPA axis, signal transduction pathway, and other pathways. This paper also summarizes and analyzes the existing problems of traditional Chinese medicine polysaccharide and the next research focus, in order to provide scientific basis and new ideas for the research and development of the target and mechanism of depression treatment and the effective components of traditional Chinese medicine polysaccharide.

43 Research progress on pharmacological mechanism of Xinkai Kujiang classic prescriptions intervention in the treatment of ulcerative colitis

The pathogenesis of cold and heat interspersed is throughout ulcerative colitis, it’s should be treated with Xinkai Kujiang. Banxia Xiexin Decoction, Gancao Xiexin Decoction, Wumei Pills, and other related prescriptions can play a good curative effect when used, especially in relieving clinical symptoms, improve patients’ quality of life and so on. However, the pharmacological mechanism is still unclear. It may regulate the inflammatory balance by inhibiting the NLRP3 inflammatorome, IL-6/STAT3 signaling pathway, and TLR4/NF-κB signaling pathway; or regulate intestinal mucosal biological barrier; or inhibition of excessive apoptosis. In this paper, the mechanism of Xinkai Kujiang treatment for ulcerative colitis has been reviewed in recent years, so as to provide theoretical basis for the good effect of traditional Chinese medicine on ulcerative colitis and point out the direction for further drug research.

47 Research progress on the role of mast cells in knee osteoarthritis in obese patients

Mast cells (MCs) are immune cells in human tissues, and obesity is one of the risk factors for knee osteoarthritis (KOA). MCs have important physiological and pathological effects in adipose tissue, and the two influence each other and play an important role in the progression of KOA in obese patients. In this paper, the activation of MCs, the obesity-induced inflammatory factors of KOA, the role of MCs in obese tissue, the influence of MCs-related mediators on KOA in obese patients, and the signaling pathways related to KOA in obese patients are summarized and analyzed, and the role of MCs in KOA in obese patients is summarized, in order to provide new ideas for the diagnosis, treatment, and prevention of KOA in obese patients, it may help in the search for prevention targets and therapeutic drug targets.

51 Comparison of different anticoagulant therapy strategies in pregnancy after mechanical heart valve replacement surgery

The hypercoagulable state during pregnancy increases the risk of thromboembolism in pregnant women after mechanical heart valve replacement surgery, and effective anticoagulant therapy is very important. Warfarin can effectively prevent valve thrombosis, which is the safest for the mother, but can cause embryopathy. Unfractionated heparin and low molecular weight heparin are safe for the fetus, but have weak antithrombotic effects and increase the risk of maternal valve thrombosis. At present, there are few studies on the use of new oral anticoagulants in patients after mechanical heart valve replacement surgery, and the existing evidence is not recommended for such populations. With the deepening of research, for patients with high embolism risk who can reach the target international normalized ratio by using low dose Warfarin (less than or equal to 5 mg/d), Warfarin may be considered throughout pregnancy. If high dose Warfarin (greater than 5 mg/d) is used to achieve the target international normalized ratio, low molecular weight heparin with adjusted dosage can be used in the early pregnancy, and Warfarin can be used in the middle and late pregnancy.

56 Research advances in the role of pyroptosis in neonatal hypoxic-ischemic  encephalopathy

Hypoxic-ischemic encephalopathy (HIE) is the leading cause of disability and death in neonatal, which has been a major health concern worldwide. At present, there is still a lack of satisfactory treatment for HIE, which is a great threat to children’s health and quality of life. Mutiple researches have shown that the activation and diffusion of inflammation is the main reason for the occurrence and development of HIE. Pyroptosis is a relatively recently described mechanism of inflammatory cell death that is closely related to HIE. The study on pyroptosis and HIE is increasing year by year. This paper summarizes the pivotal role of pyroptosis in HIE and analysis the related mechanisms to provide a basis for future research.

61 Research progress on the correlation between fibrinogen and diabetic nephropathy

Diabetic nephropathy (DN) is one of the chronic complications of diabetes mellitus, which can easily lead to renal failure and finally develop into end-stage renal disease. Fibrinogen (FIB) is an important substance before thrombosis, and its elevated level indicates hypercoagulation. With the progression of DN, the risk of thrombosis increases and cardiovascular disease is easy to be complicated. The occurrence of cardiovascular events is one of the causes of death in DN patients. This paper reviews recent studies on the correlation between FIB and DN, aiming to explore the current research status of the relationship between FIB and DN, to elaborate the important clinical significance of detecting FIB in the diagnosis and treatment of DN patients, and to provide support for preventing thrombosis and reducing the risk of cardiovascular events in patients with DN.

65 Study of aggressive behavior and its influencing factors in adolescents with depression

Objective To explore the aggressive behavior and its influencing factors in adolescents with depression. Methods A total of 195 adolescent patients with depression who first visited the First Hospital of Hebei Medical University from January 2022 to June 2022 were selected as the research objects. Aggressive questionnaire (AQ) was used to evaluate the aggressive behavior of the patients, and self-rating depression scale (SDS) was used to evaluate the severity of depression. The aggressive behavior of adolescents with different demographic data and different severity were compared and analyzed, and the risk factors of aggressive behavior in adolescents with depression were analyzed by multiple linear regression method. Results There were statistically significant differences in AQ scores among patients with different gender, residence, one-child status, parenting style, and family history of mental illness (P<0.05). Female, urban, only child, violent, and positive family history of mental illness had higher AQ scores (P<0.05). There were statistically significant differences in physical aggression, verbal aggression, hostility and AQ score among patients with different severity (P<0.05). The physical aggression, verbal aggression, and AQ scores of severe group were higher than those of mild and moderate groups, and the hostility Score of severe group was higher than that of mild group, and the differences were statistically significant (P<0.05). There was no significant difference in anger score among different severity group (P>0.05). Multiple regression analysis showed that female, violent parenting style, and depression severity were risk factors for aggressive behavior (P<0.05). The regression equation was as follows: Y=83.009+5.640X1+8.306X2+9.516X3. Conclusion Female, living in urban areas, only child, violent family rearing style, and major depression adolescents with depression are more likely to appear aggressive behavior, among which female, violent family rearing style, and major depression are risk factors for aggressive behavior.

Objective To explore the correlation between self-efficacy and dietary self-management in patients with nephrotic syndrome (NS). Methods A total of 110 NS patients admitted to Huaian First People’s Hospital in Jiangsu Province from January 2019 to February 2021 were randomly selected as research objects. Demographic data, scores of general self-efficacy scale (GSES), and scores of total score and diet-related self-management items were collected by questionnaire after admission. Patients were divided into two groups according to GSES scores. The low performance group was included in the GSES score ≤25 points, and the high performance group was included in the GSES score >25 points. The scores and total scores of each item of diet-related self-management were observed in the two groups, and the correlation between patients’ self-efficacy and the level of diet self-management was analyzed. Results There were significant differences in education level and family support between the two groups (P<0.05). The scores and total scores of each item of diet-related self-management ability in high-performance group were higher than those in control group, and the differences were statistically significant (P<0.05). The correlation coefficients ρ of each item of patients’ self-efficacy and diet-related self-management ability were 0.350-0.923, respectively (P<0.05). Conclusion There is a certain relationship between the self-efficacy of NS patients and the level of dietary self-management. Clinical improvement of patients’ self-efficacy can promote the improvement of their dietary self-management level, which can provide guarantee for patients’ disease control.

73 Association of serum miR-137 and Notch1 expression with diabetic retinopathy

Objective To investigate the relationship between serum microRNA-137 (miR-137) and Notch receptor 1 (Notch1) expression with diabetic retinopathy (DR). Methods A total of 147 type 2 diabetes mellitus (T2DM) patients admitted to the First Central Hospital of Baoding, Hebei Province (hereinafter referred to as “our hospital”) from January 2021 to December 2022 were selected as T2DM group, which were divided into 45 patients in the DR group and 102 patients in the non-DR group according to whether they had concurrent DR, and 15 patients in the hyperplastic stage group, and 30 patients in the non-hyperplastic stage group according to the DR stage. Another 52 healthy subjects in our hospital during the same period were selected as the control group. Clinical data of T2DM patients were collected, and serum miR-137 and Notch1 mRNA expressions were detected by real-time fluorescence quantitative polymerase chain reaction. Pearson correlation coefficient was used to analyze the correlation between serum miR-137 and Notch1 mRNA expression in patients with DR, and the influencing factors for the occurrence of patients with T2DM complicated with DR were analyzed. Receiver operating characteristic (ROC) curve was used to analyze the predictive value of serum miR-137, and Notch1 mRNA expression in patients with T2DM. Results Serum miR-137 expression was increased and Notch1 mRNA expression was decreased in the T2DM group compared with the control group (P<0.05). Serum miR-137 expression was increased and Notch1 mRNA expression was decreased in the DR group compared with the non-DR group (P<0.05). Serum miR-137 was negatively correlated with Notch1 mRNA expression in DR patients (r=-0.780, P<0.001). Serum miR-137 expression was increased and Notch1 mRNA expression was decreased in the proliferative group compared with the non-proliferative group (P<0.05). Multifactorial logistic regression analysis showed that prolonged T2DM disease, elevated glocosglated hemoglobin albumin, and miR-137 ≥1.47 were independent risk factors for patients with T2DM complicated with DR, and Notch1 mRNA ≥0.98 was an independent protective factor[OR (95%CI)=1.181 (1.059-1.318), 1.613 (1.170-2.224), 3.550 (0.734-0.958), and 0.188 (0.071-0.498)]. ROC curve analysis showed that the area under the curve of serum miR-137 and Notch1 mRNA expression combined to predict concurrent DR in T2DM patients was greater than that predicted by miR-137 and Notch1 mRNA alone (P<0.05). Conclusion High serum miR-137 expression and low Notch1 expression in T2DM patients are closely associated with DR and may be an auxiliary predictor of DR.

Objective To investigate the effect of corn silk aqueous extract on glucose and lipid metabolism, liver and kidney function in newly diagnosed mild type2 diabetes mellitus (T2DM) elderly patients. Methods A total of 72 newly diagnosed mild T2DM elderly patients were recruited from the Second Affiliated Hospital of Qiqihar Medical College from June to October 2020, and they were divided into intervention group and control group by random number table method, with 36 cases in each group. The patients in the intervention group took 4.8 g corn silk aqueous extract every day, and the control group was given the same amount of toast powder as placebo. The indexes of glucose and lipid metabolism, liver, and kidney function were compared between the two groups. Results After treatment, fasting insulin, insulin resistance index, alanine aminotransferase, and aspartate aminotransferase in the intervention group were lower than before treatment, insulin sensitivity index, and total bilirubin were higher than before treatment, fasting blood glucose in the control group was higher than before treatment, and fasting blood glucose, fasting insulin, insulin resistance index, total cholesterol, aspartate aminotransferase and total bilirubin in the intervention group were lower than before treatment, insulin sensitivity index was higher than control group, the differences were statistically significant (P<0.05). Conclusion The intervention of corn silk aqueous extract can improve fasting blood glucose and insulin resistance, regulate blood lipid, have no adverse effect on liver and kidney function in elderly patients with newly diagnosed mild T2DM.

83 Variation and correlation of ultrasound measurements in non-insertional achilles tendinopathy

Objective To investigate the changes and correlation of ultrasound measurements of achilles tendon lesions in patients with non-insertional achilles tendinopathy. Methods A total of 55 patients with non-insertional achilles tendinopathy (NIAT) was NIAT group and 20 healthy adults was lualthy group of achilles tendon in the Third Affiliated Hospital of Beijing University of Chinese Medicine from November 2020 to August 2022 were collected. Musculobone ultrasound (MSK-US) and color Doppler flow imaging (CDFI) were used to measure the thickness and cross-sectional area of the achilles tendon and the thickness of the Kager fat pad in front of the achilles tendon, and the deepening saceffusion of the achilles tendon and the blood flow signals in the achilles tendon were explored, and the changes in the measured values of the Achilles tendon between the two groups were compared. Combined with visual analogue scale (VAS) and American Collgege of Foot and Ankle Surgery (AOFAS) scale, Pearson correlation analysis was used to explore the correlation of ultrasonic performance achilles tendon with VAS and AOFAS scores. Results The achilles tendon thickness, cross-sectional area, Kager fat pad thicknessin NIAT group were higher than those in healthy group, and the detection rate of deep bursae effusion and blood flow signal in NIAT group was higher than that in healthy group, with statistical significance (P<0.05). Achilles tendon thickness and achilles tendon blood flow detection rate were positively correlated with VAS score (r=0.249, 0.334, P<0.05), and blood flow signal was negatively correlated with AOFAS score (r=-0.309, P<0.05). Conclusion Patients with non-insertional achilles tendinopathy may develop lesions of achilles tendon and peri-tendon tissues. The thickness of achilles tendon and the detection rate of blood flow signal in achilles tendon are important indicators to evaluate the condition of achilles tendinopathy. MSK-US and CDFI play an important role in the evaluation of achilles tendinopathy.

87 Effect of tourniquet usage on rehabilitation after unicompartmental knee arthroplasty

Objective To investigate the effect of tourniquet usage on rehabilitation after unicompartmental knee arthroplasty. Methods A retrospective analysis was performed on 100 patients who received unicondylar replacement of knee joint in the Department of Joint Surgery, the Affiliated Hospital of Xuzhou Medical University from January to December 2021. According to the way of tourniquet application, the patients were divided into the whole course group of 33 cases, the second half group of 34 cases, and the non-tourniquet group of 33 cases. Operation time, blood loss, hemoglobin (Hb) and erythrocyte volume (Ht), thromboelasticity chart, visual analog scale (VAS) score, swelling rate of affected limb, American hospital for special surgery (HSS) score of knee joint and postoperative complications were recorded in the three groups. Results The swelling rate of the whole course group was higher than that of the second half group and non-tourniquet group (P<0.05). At 24 h after surgery, Hb and Ht in the three groups were lower than before surgery, and the whole course group was higher than the second half of the course group and non- tourniquet group (P<0.05). The VAS score of the whole course group was higher than that of the second half group and non-tourniquet group, the total blood loss and intraoperative blood loss were less than those of the second half group and non-tourniquet group, and the recessive blood loss was more than those of the second half group and non-tourniquet group (P<0.05). The operation time of the whole course group and the second half group was shorter than that of the group without tourniquet (P<0.05). The overall analysis showed that there were significant differences between groups and time points in HSS evaluation (P<0.05). Further pairwise comparison and intra-group comparison: HSS scores of the three groups were compared at each time point, and the differences were statistically significant (P<0.05). Comparison between groups: HSS score one month after operation in the whole course group was lower than that in the second half group and non-tourniquet group (P<0.05). The overall analysis showed that there were significant differences among MA groups in comparison, time point comparison and interaction (P<0.05). For further pairwise comparison, intra-group comparison showed that the MA of the whole group before surgery was lower than one day after surgery, and that of the whole group one day after surgery was higher than before surgery and seven days after surgery (P<0.05). Comparison between groups: MA of the whole course group at seven days after surgery was higher than that of the second half group and non-tourniquet group (P<0.05). There was no significant difference in the incidence of complications one month after operation among the three groups (P>0.05). Conclusion The use of a tourniquet from the completion of osteotomy to the completion of suturing during unicompartmental knee replacement helps to reduce the effect on coagulation status, reduce postoperative pain and the degree of swelling of the affected limb, shorten the operative time, and promote functional recovery.

92 Influence of early ambulation program based on ERAS concept on postoperative rehabilitation and compliance of patients with multiple rib fractures

Objective  To investigate the effect of early ambulation program based on the concept of enhanced recovery after surgery (ERAS) on postoperative rehabilitation and compliance in patients with multiple rib fractures. Methods A total of 98 patients with multiple rib fractures who were admitted to Shanghai Sixth People’s Hospital from January to September 2022 were selected, and they were divided into ERAS group and routine group by random number table method, with 49 cases in each group. The routine group was given routine nursing intervention, the ERAS group was given early ambulation program based on the ERAS concept on the basis of control group. The postoperative drainage volume, drainage tube removal time, hospital stay, NRS score after surgery, and compliance were compared between the two groups. Results The postoperative drainage volume in ERAS group was less than that in conventional group, and the time of drainage tube removal and hospital stay in ERAS group were shorter than those in conventional group, with statistical significances(P<0.05). There were statistically significant differences in NRS score point comparison, inter-group comparison and interaction between the two groups (P<0.05). Intra-group comparison: NRS scores of the two groups were compared at each time point, and the differences were statistically significant (P<0.05). Comparison between groups: NRS score of ERAS group at each time point was lower than that of conventional group, and the difference was statistically significant (P<0.05). The compliance of ERAS group was better than that of conventional group, with statistical significance (P<0.05). Conclusion  The early ambulation program based on ERAS concept is beneficial to relieve postoperative pain in patients with multiple rib fractures, promote postoperative recovery, and improve patient compliance, which is worthy of promotion.

96 Retrospective analysis of clinical features of 36 children with brain abscess

Objective To analyze the etiology, clinical features, imaging features, and treatment and prognosis of brain abscess in children, and to further understand brain abscess. Methods A total of 36 hospitalized children with definite diagnosis of pediatric brain abscess admitted to the Department of Pediatrics and Neurosurgery of Beijing Tiantan Hospital, Capital Medical University from January 2014 to December 2021 were selected. The distribution at different ages, clinical manifestations, etiological examination, imaging examination, peripheral blood/pus culture, parasitic infection, etiological analysis, and treatment and prognosis of brain abscess in children were observed. Results The age<11 years of onset of pediatric brain abscess was 24 cases. The most common neurological manifestations of pediatric brain abscess were headache, followed by fever and vomiting. The most commonly affected site was the frontotemporal lobe, which was dominated by single abscess. In peripheral blood culture examination, only one case was positive; among the 28 cases of pus culture, 12 cases were positive, mainly streptococcus and staphylococcus. No parasitic infection was found in the 36 children. The etiology was clear in 23 of the 36 cases, otitis media was the main cause, followed by brain trauma and congenital heart disease. A total of 28 cases underwent surgical treatment and eight cases received conservative medical treatment. Thirty-one of the 36 cases were cured and discharged from hospital, two cases had sequelae, and three cases gave up treatment. By December 2021, no abscess recurrence was observed and three patients died. Conclusion Early childhood is the peak of pediatric brain abscess, headache is the most common clinical manifestations. Otitis media, heart disease, and brain trauma are the main causes of brain abscess, frontotemporal lobe abscess is more common, the positive rate of pus culture is higher than that of blood culture, streptococcus mostly.

100 Application of umbilical artery blood gas analysis combined with ultrasonic Tei index in the diagnosis of neonatal asphyxia

Objective To study the diagnostic value of umbilical artery blood gas analysis combined with ultrasonic Tei index in neonatal asphyxia. Methods Eighty cases of neonatal asphyxia patients from Jiangsu Shengze Hospital Affiliated to Nanjing Medical University (“our hospital”for short) from April 2019 to March 2022 were selected as asphyxia group, in the same period, 80 normal newborns of our hospital were selected as the control group. Umbilical artery blood gas indexes (hydrogenion concentration [pH], base excess (BE), arterial carbon dioxide pressure (PaCO2)], and ultrasonic Tei index were compared between two groups; the influencing factors of neonatal asphyxia were analyzed by logistic regression; and the diagnostic value of umbilical artery blood gas indexes combined with ultrasonic Tei index in neonatal asphyxia was determined by receiver operating characteristic curve. Results There were statistically significant differences in anemia during pregnancy, Apgar score 1 min after birth, oligohydramnios, umbilical cord around neck, and intrauterine distress between two groups (P<0.05). The pH and BE of asphyxia group were lower than those of control group, PaCO2 and ultrasonic Tei index were higher than those of control group (P<0.05). Oligohydramnios, intrauterine distress, pH, BE, PaCO2, and ultrasonic Tei index were the influencing factors of neonatal asphyxia (P< 0.05). The area under the curve of pH, BE, PaCO2, and ultrasonic Tei index combined detection was higher than that of each single detection. Conclusion Umbilical artery blood gas analysis combined with ultrasonic Tei index has high diagnostic value for neonatal asphyxia, and has certain clinical guiding significance.

104 Relationship between plasma albumin, free protein S, and D-dimer and onset of preeclampsia

Objective To investigate the relationship between plasma albumin (PA), free protein S (fPS), and D-dimer (D-D) and onset of preeclampsia (PE). Methods A total of 112 patients with PE who underwent regular obstetric examinations in Liyang People’s Hospital of Jangsu Province (“our hospital”for short) from January 2020 to December 2021 were selected as PE group, and 120 healthy pregnant women who underwent obstetric inspection in our hospital during the same period were selected as control group. The general data and clinical data, plasma PA, fPS, and D-D were compared between two groups; the influencing factors of onset of PE were analyzed by logistic regression analysis; the predictive value of plasma PA, fPS, and D-D to the onset of PE was determined by receiver operating characteristic curve. Results The levels of systolic blood pressure, diastolic blood pressure, urine protein, serum uric acid, fibrinogen, homocysteine, high-sensitivity C-reactive protein, and D-D in PE group were higher than those in control group; while activated partial thrombin time , plasma PA, and fPS were lower than those in control group (P<0.05). Fibrinogen (OR=2.855), PA (OR=0.314), fPS (OR= 0.297), and D-D (OR=3.053) were the influencing factors of onset of PE (P<0.05). The area under the curve of the combined detection of PA, fPS, and D-D was higher than that of the independent detection of each index. Conclusion Plasma PA, fPS, and D-D are related to the incidence of PE, and the combination detection of plasma PA, fPS, and D-D is of high diagnostic value for PE.

108 Analysis of genetic factors in missed abortion by high-throughput sequencing technique

Objective To analyze the genetic factors in missed abortion villi tissue by high-throughput sequencing technique. Methods A retrospective analysis was performed on 91 patients who underwent palace clearing operation due to missed abortion in the First Affiliated Hospital of Tsinghua University from June 2017 to June 2020. The abortive villi tissue was analyzed by high-throughput sequencing, and the changes of villus chromosome number and copy number variation were analyzed. Results High-throughput sequencing was performed on the missed abortion villi tissue of 91 cases. A total of 70 cases (76.08%) had chromosome abnormality, including 39 cases with abnormal chromosome number, 5 cases with chimera, and 26 cases with copy number variation. There were 14 cases of micro duplication, 6 cases of micro deletion, and 6 cases of mixed type. Conclusion Chromosome abnormality of embryo is the main cause of missed abortion, mainly aneuploidy. High-throughput sequencing copy number variation is applied to the detection of villi chromosomes, which has high sensitivity, can increase the detection rate of chromosomal abnormalities, and is helpful to clarify the cause of missed abortion.

112 Study of changes of serum homocysteine level in patients with central retinal vein occlusion and its relationship with vascular endothelial function

Objective To analyze the changes of serum homocysteine (Hcy) level and its relationship with vascular endothelial function in patients with central retinal vein occlusion (CRVO). Methods A total of 120 patients with CRVO admitted to the Department of Ophthalmology of the First Hospital of Zhangjiakou City of Hebei Province (hereinafter referred to as “our hospital”) from January 2020 to January 2022 were selected as the observation group, and 115 healthy eye subjects from the physical examination center of our hospital from January 2020 to January 2022 were selected as the comparison group. The levels of serum Hcy, blood lipid indexes, nitric oxide (NO), and endothelin-1 (ET-1) were detected and compared between the two groups. According to the results of serum Hcy examination, the observation group was further divided into severe elevation group (26 cases) (>100 μmol/L), moderate elevation group (50 cases) (30-100 μmol/L), and mild elevation group (44 cases) (16-<30 μmol/L). The blood lipid indexes, NO, and ET-1 levels of the three groups were compared. Pearson correlation coefficient was used to analyze the relationship between serum Hcy and blood lipid indexes, NO, and ET-1 in the observation group. Results The serum Hcy, ET-1, low-density lipoprotein cholesterol (LDL-C), total cholesterol (TC), and triglyceride (TG) in the observation group were higher than those in the comparison group, and the high-density lipoprotein cholesterol (HDL-C) and NO were lower than those in the comparison group, and the differences were statistically significant (P<0.05). The levels of ET-1, LDL-C, TC, and TG in the severe elevation group were higher than those in the moderate and the mild elevation group, and the levels of HDL-C and NO were lower than those in the moderate and the mild elevation group, and the differences were statistically significant (P<0.05). The levels of ET-1, LDL-C, TC, and TG in the moderate elevation group were higher than those in the mild elevation group, and the levels of HDL-C and NO were lower than those in the mild elevation group, and the differences were statistically significant (P<0.05). In the observation group, serum Hcy was positively correlated with ET-1, LDL-C, TC, and TG (r>0, P<0.05), and negatively correlated with HDL-C and NO (r<0, P<0.05). Conclusion The serum Hcy level of CRVO patients is significantly higher than that of healthy people, and the serum Hcy level is correlated with blood lipids and the degree of vascular endothelial function damage.

116 Effect of Kangfuxin Liquid on patients with moderate chronic periodontitis

Objective To investigate the effect of Kangfuxin Liquid on patients with moderate chronic periodontitis. Methods A total of 80 patients with moderate chronic periodontitis admitted to the First Affiliated Hospital of Xinjiang Medical University from September 2020 to December 2021 were selected and divided into treatment A group (40 cases) and treatment B group (40 cases) by random number table method. The treatment A group was treated with scaling and root planning (SRP) and Kangfuxin Liquid, and the treatment B group was treated with SRP and Xipayi Guyin Ye. Both groups were treated continuously for one month. The changes of periodontal indexes, inflammatory factors, and oxidative stress indexes were compared between the two groups before and after treatment. Results After treatment, the plaque index, sulcus bleeding index, probing depth, clinical attachment loss level, tumor necrosis factor-α, interleukin-6, and malondialdehyde in the two groups were lower than those before treatment, and those in the treatment A group were lower than those in the treatment B group, and the differences were statistically significant (P<0.05). Conclusion Kangfuxin Liquid combined with SRP in the treatment of moderate chronic periodontitis can improve the periodontal indexes of patients, reduce the inflammatory response and oxidative stress response of periodontal tissue, and promote the recovery of periodontal tissue. The effect is better than Xipayi Guyin Ye.

Objective To study the content determination method of Fuzheng Shengxue Capsules and to investigate its stability. Methods The contents of polydatin and resveratrol in Fuzheng Shengxue Capsules were determined by high performance liquid chromatograph. Agilent YMC-Triart C18 (250 mm×4.6 mm, 5 μm) was used as the column, and acetonitrile∶water (25∶75) was used as the eluent; elution time was 15 min; the wavelength was 312 nm, the flow rate was 1.1 ml/min, the column temperature was 27℃, and the sample size was 10 μl. The stability of Fuzheng Shengxue Capsules was investigated according to the 2020 edition of Pharmacopoeia of the People’s Republic of China. Results The linear relationships of polydatin and resveratrol were good in the concentration ranges of 16.81-168.81 μg/ml and 4.52-45.23 μg/ml, respectively, and the regression equations were Y=115.2X+101.91 (r=0.999 9), Y=2.75X+28.59 (r=0.999 9) respectively. The average recoveries were 99.23% and 99.84%. The contents of polydatin and resveratrol were 551.0-555.3 μg/g and 136.9-140.4 μg/g, respectively. Fuzheng Shengxue Capsules was stable in two years. Conclusion The content determination method of the preparation is simple, easy, and repeatable; and the validity period is tentatively set to be two years.

This article summarizes Professor Gao Zhan’s experience in the treatment of prostate cancer and its complications based on the theory of “prevent progression after disease onset”. Professor Gao Zhan believes that for prostate cancer, first of all, for hormone-sensitive prostate cancer, it is appropriate to eliminate pathogenic factor as the main, strengthening vital qi as a supplement, clearing heat, promoting blood, and detoxification as the main, supplemented by supplementing qi and replenishing the spleen to prolong the period; for castration-resistant prostate cancer, strengthening vital qi as the main, eliminate pathogenic factor as a supplement, and clearing heat, activating blood, and detoxification, delay systemic metastasis, prolong survival. Secondly, for the related complications after radical prostatectomy of prostate cancer, such as postoperative urinary incontinence, metabolic complications after androgen deprivation treatment, and adverse reactions during perichemotherapy, the traditional Chinese medicine treatment based on the theory of “prevent progression after disease onset” can significantly control symptoms and improve adverse reactions. In clinical practice, Professor Gao Zhan selects reasonable treatment plans for patients at different stages of disease and covering complications according to the theory of “prevent progression after disease onset”, which has an important guiding role in the prevention and treatment of prostate cancer.

Professor Wang Xuanzhang is very expert in the treatment of soft tissue injury and internal and gynecological diseases by massage, with rich experience and considerable achievements. After years of research and practice on classics such as Huangdi Neijing, Professor Wang Xuanzhang put forward the theory of “theory, method, prescription, technique” of massage, such as “truth theory”, “method of acupoint-pressing and pulse-regulating”, “differentiation method of form and qi”, “method of  five-level massage” and the classification of five elements and yin yang of massage manipulation, guiding the clinical diagnosis and treatment of massage with significant effect and providing new ideas and methods for massage teaching and clinical practice. It plays an important guiding role in the innovation, development, and research of massage. The academic theory and experience of massage of professor Wang Xuanzhang are worthy of research, reference, and inheritance by future generations of doctors.

139 Rehabilitation effect of graded cooling nursing under early warning assessment on children with febrile convulsion

Objective To explore the rehabilitation effect of graded cooling nursing under early warning assessment for children with febrile convulsion. Methods A total of 194 children with febrile convulsion treated in the Second People’s Hospital of Lu’an City, Anhui Province from January 2019 to December 2021 were selected and divided into control group and observation group by random number table method, with 97 cases in each group. The control group was given routine cooling nursing, and the observation group was given graded cooling nursing under early warning assessment on the basis of the control group. After one week of intervention, the clinical symptoms, incidence of complications, and immune function were compared between the two groups. Results The duration of high fever and convulsion in the observation group were shorter than those in the control group, and the frequency of convulsion was lower than that in the control group (P<0.05). The total incidence of complications in the observation group was lower than that in the control group (P<0.05). After intervention, the levels of immunoglobulin (Ig) A, IgM, IgG of the two groups were lower than those before intervention, and those of the observation group were lower than those of the control group (P<0.05). After intervention, the scores of physiological function, emotional function, social function, and school function of pediatric quality of life inventory 4.0 in two groups were higher than those before intervention, and those of the observation group were higher than those of the control group (P<0.05). Conclusion Graded cooling nursing under early warning assessment can improve the symptoms of high fever and convulsion in children with febrile convulsion, reduce the incidence of complications, and improve the immune function and quality of life.

Objective To explore the application of self-adhesive soft silicone foam dressing(Mepilex Lite) combined with 3M transparent dressing in nursing care of patients with cirrhotic ascites. Methods This study used a prospective randomized controlled trial. A total of 100 patients with cirrhosis ascites who underwent peritoneal catheterization in the Department of Gastroenterology, the First Affiliated Hospital of Guangxi Medical University from January 2020 to June 2022 were included. According to random number table method, the patients were divided into intervention group and control group, with 50 cases in each group. In the intervention group, Mepilex Lite combined with 3M transparent dressing was used to cover the puncture point of abdominal catheterization after abdominal catheterization, in the control group, only 3M transparent dressing was used to cover the puncture point after abdominal catheterization. The occurrence of adverse reactions at puncture points, such as body temperature, local bleeding and fluid exudation at puncture points, subcutaneous hematoma, tube slippage, pain degree during excipient replacement, dressing replacement frequency, patients’ subjective feelings and tube maintenance cost were compared between the two groups. Results The body temperature and visual analog score of the intervention group were lower than that of the control group, and the pipeline maintenance cost was lower than that of the control group, the differences were statistically significant (P<0.05). The dressing changing times in the intervention group was lower than that in the control group, and the dressing changing interval was longer than that in the control group, the difference was statistically significant (P<0.05). Comfort and nursing satisfaction of intervention group were better than control group, the differences were statistically significant (P<0.05). The total incidence of adverse reactions in the intervention group was lower than that in the control group, and the difference was statistically significant (P<0.05). Conclusion The application of Mepilex Lite combined with 3M transparent dressing in patients with ascites of cirrhosis with intraperitoneal catheterization is remarkable. The incidence of adverse reactions such as local exudation, exudation and subcutaneous hematoma is lower, the body temperature remained stable, and the degree of pain was reduced, comfort and satisfaction of nursing was higher.

Objective To explore the effect of Satya model nursing intervention on mental state and self-efficacy of breast cancer patients. Methods A total of 200 patients undergoing breast cancer treatment in Jiangsu Provincial People’s Hospital from January 2020 to December 2021 were selected. By random number table method, they were divided into control group and study group, with 100 cases in each group. The control group received routine nursing, and the study group received nursing intervention based on Satya model on the basis of the control group. The mental health continuum-short form (MHC-SF) and general self-efficacy scale (GSES) scores of the two groups were compared before and after intervention. Results After intervention, MHC-SF scores of both groups were lower than before intervention, and the study group was lower than the control group, and the differences were statistically significant (P<0.05). After intervention, GSES scores in both groups were higher than those before intervention, and the study group was higher than the control group, and the differences were statistically significant (P<0.05). Conclusion For breast cancer patients, the implementation of nursing intervention based on the Satya model can effectively improve the psychological state of patients and enhance self-efficacy.

151 Application effect of health education based on mind map in patients undergoing thoracoscopic pulmonary nodule resection

Objective To explore the application effect of health education based on mind map in patients undergoing thoracoscopic pulmonary nodule resection. Methods A total of 80 patients with thoracoscopic pulmonary nodule resection admitted to Mingguang People’s Hospital of Anhui Province from March 2020 to June 2022 were selected. According to the random number table method, they were divided into control group and observation group, with 40 cases in each group. The control group was given routine health education, and the observation group was given health education based on mind map. The self-efficacy, active cough ability, lung function, and postoperative rehabilitation process were observed in the two groups. Results One month after operation, the general self-efficacy scale score and levels of forced vital capacity, forced expiratory volume in one second, and forced expiratory volume in one second/forced vital capacity ratio in the two groups were higher than those on admission, and those of the observation group were higher than those of the control group, the differences were statistically significant (P<0.05). One day after operation, the classification of active cough ability in the observation group was better than that in the control group, and the difference was statistically significant (P<0.05). The first anal exhaust time, the first defecation time, and hospitalization time of the observation group were shorter than those of the control group, and the differences were statistically significant (P<0.05). Conclusion Health education based on mind map can improve the self-efficacy, active cough ability and lung function of patients undergoing thoracoscopic pulmonary nodule resection, and can shorten their rehabilitation process.

156 Preventive effect of postoperative nursing based on quality control circle on deep venous thrombosis of lower limbs after lower limb fracture surgery

Objective To research the preventive effect of postoperative nursing based on quality control circle (QCC) on lower limb deep vein thrombosis (DVT) after lower limb fracture operation. Methods A total of 90 patients with lower limb fracture who received surgical treatment in Anqing First People’s Hospital, Anhui Province from January 2021 to March 2022 were selected, according to random number table method, the patients were divided into control group and observation group, with 45 cases in each group. The control group received conventional perioperative nursing intervention, and the observation group received postoperative nursing intervention based on QCC. The incidence of postoperative DVT, Homan sign and Neuhof sign, postoperative femoral vein and popliteal vein blood flow rate, and the comprehensive quality assessment results of the two groups of patients to the nursing staff were compared. Results The incidence of postoperative DVT, Homan sign, and Neuhof sign in the observation group was significantly lower than that in the control group, and the differences were statistically significant (P<0.05). Overall analysis found that there were statistically significant differences in blood flow velocity, time point, and interaction between the two groups of femoral vein and popliteal vein (P<0.05). Intra-group comparison: Blood flow velocity of femoral vein, and popliteal vein in the two groups on postoperative day 1, 7, and 14 was statistically significant (P<0.05). Comparison between groups: Blood flow velocity of femoral vein and popliteal vein in the observation group was significantly higher than that in the control group at 7 and 14 days after surgery, and the differences were statistically significant (P<0.05). After intervention, the scores of problem solving ability, communication and coordination ability, work enthusiasm, team sense and responsibility of nursing staff in the observation group were significantly higher than those in the control group, and the difference was statistically significant (P<0.05). Conclusion Postoperative nursing based on QCC can significantly reduce the incidence of postoperative DVT in patients with lower limb fracture and improve the comprehensive quality of nursing staff.

160 Influence of PRIDE combined with pelvic floor muscle strengthening exercise on urinary incontinence and quality of life in patients after prostatectomy

Objective To investigate the effect of PRIDE combined with pelvic floor muscle strengthening exercise in nursing patients after prostatectomy. Methods A total of 73 patients who underwent prostatectomy in Chaohu Hospital, Anhui Medical University from January 2020 to May 2022 were selected and divided into the reference group and the intervention group by random number table method. The reference group (36 cases) was given pelvic floor muscle strengthening exercise, and the intervention group (37 cases) was given PRIDE program intervention on the basis of the reference group. Seven days as an intervention course, two groups all received a total of four courses of intervention. The changes of urinary incontinence and quality of life related scales were observed before and after the intervention. Results After the intervention, the scores of behavior management, maintenance, confidence, and total scores of self-care scale of the two groups were higher than those before the intervention, and those of the intervention group were higher than those of the reference group, and the differences were statistically significant (P<0.05). After the intervention, the total incidence of postoperative urinary incontinence and the total incidence of complications in the intervention group were lower than those in the reference group, and the differences were statistically significant (P<0.05). After intervention, the scores of social activity limitation, behavioral limitation, psychological impact, and total score of the urinary incontinence quality of life scale of the two groups were higher than those before intervention, and those of the intervention group were higher than those of the reference group, and the differences were statistically significant (P<0.05). Conclusion Pelvic floor muscle strengthening exercise and PRIDE program can help patients improve self-management ability, reduce postoperative urinary incontinence, and other complications, and improve the quality of life.

164 Non-Kawasaki disease febrile illness complicated coronary artery aneurysm in two cases and the literature review

Kawasaki disease is not the only disease that causes coronary artery lesions (CAL) in children. Takayasu arteritis (TA) and systemic onset juvenile idiopathic arthritis (SOJIA) can also involve coronary arteries, which are relatively rare in clinic and easy to miss/misdiagnose. This paper reports a case of fever with coronary artery aneurysm (CAA) who was not well treated with intravenous immunogloblin (IVIG). High blood pressure, subclavian artery stenosis, and axillary artery aneurysm were found in the course of diagnosis and treatment. TA was finally diagnosed and relieved after treatment with Prednisone, Methotrexate, and Tocilizumab. This paper also reports a patient with SOJIA, who was hospitalized mainly due to fever and CAA. The efficacy of IVIG was poor, and then rash and multiple joint effusion (hip, knee, and shoulder joints) occurred. The patient was diagnosed as SOJIA and improved after treatment with Naproxen Tablets, Hormones and Tocilizumab. This article retrospectively reviewed the clinical features of two cases of CAA caused by non-Kawasaki disease febrile disease, in order to improve the understanding of clinical doctors of the diseases related to CAL in children.

169 Anti-N-methyl-D-aspartate receptor encephalitis combined with mediastinal teratoma: a case report and literature review

Anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis is a complex autoimmune neuropsychiatric syndrome. Previous literature has reported that anti-NMDAR encephalitis is associated with female ovarian teratoma, however, anti-NMDAR encephalitis combined with mediastinal teratoma is rare. The clinical characteristics are not clear, and the treatment has certain limitations. This article present a 14-year-old male patient with anti-NMDAR encephalitis and mediastinal teratoma. During the course of the disease, there were persistent convulsions of the limbs and involuntary movements around the mouth. Hormone shock therapy, plasma exchange, proglobulin, rituximab, and a variety of antiepileptic drugs were used in combination, poor effect. Resection of mediastinal teratoma, condition improved after discharge. In this paper, the pathogenesis, clinical manifestations, and treatment of anti-NMDAR encephalitis complicated with teratoma were reviewed based on medical records and literature, in order to improve clinicians’ awareness of this disease.

174 A case of giant esophageal gastrointestinal stromal tumor and literature review

Gastrointestinal stromal tumor (GIST) are a relatively rare of gastrointestinal diseases, and oesophageal GIST is even rarer. A case of giant esophageal stromal tumor admitted to the First People’s Hospital of Neijiang, Sichuan Province (hereinafter referred to as “our hospital”) was retrospectively analyzed, the tumor size was 10.7 cm×7.9 cm×1.6 cm. The main symptom of the patient was dysphagia obstruction, which was misdiagnosed as “advanced esophageal cancer” in the past and failed to receive timely surgical treatment, the patient was successfully treated by right thoracic and abdominal incision (Ivor Lewis surgery) in the Department of Cardiothoracic Surgery of our hospital. The postoperative pathological diagnosis was GIST. According to the plan, the patient ate normally on the 7th day after surgery and was discharged successfully on the 10th day after surgery. Therefore, comprehensive treatment such as preoperative comprehensive evaluation, individualized surgical plan and postoperative targeted drug-assisted therapy can help improve GIST patients’ quality of life and prolong survival rate. Therefore, this paper reviewed the literature on the origin, diagnosis, and treatment of GIST in order to improve the attention of clinicians to this disease.

Objective To analyze the changes and characteristics of drug regulatory policies in different stages and provides some ideas for optimizing drug regulatory policies. Methods From 1984 to 2021, a total of 119 policy texts related to drug regulation issued by the central government were selected for quantitative analysis using content analysis method. Results China had adopted the supervisory methods of compulsory supervision, incentive supervision, ability construction supervision, and symbol persuasion supervision, but there was insufficient equilibrium. Conclusion Drug regulation should not be confined to single regulation, but should use more targeted and diversified regulatory means, forming a new situation of regulatory diversification.

Objective To explore the economic factors affecting the level of basic medical insurance for urban and rural residents in Inner Mongolia, and to provide scientific basis for the reform of basic medical insurance system for urban and rural residents in Inner Mongolia. Methods Descriptive analysis was made on the basic medical insurance level and per capita gross domestic prodict (GDP) of urban and rural residents in Inner Mongolia from January 2009 to December 2019, the output value/GDP of the tertiary industry, fiscal revenue, per capita disposable income, per capita consumption expenditure, consumer price index of residents, consumer price index of medical care and government health expenditure. The economic influencing factors of the basic medical insurance level of urban and rural residents in Inner Mongolia were analyzed by using the grey relational model. Results From 2009 to 2019, the level of basic medical insurance for urban and rural residents in Inner Mongolia and the economic influencing factors showed an overall upward trend year by year. The grey comprehensive correlation value of the basic medical insurance level of urban and rural residents from large to small was fiscal income (0.745 0), per capita disposable income (0.733 5), government health expenditure (0.726 2), per capita consumption expenditure (0.713 1), per capita GDP (0.661 0), health care consumer price index (0.584 1), tertiary industry output value/GDP (0.576 1), and consumer price index (0.569 4). Conclusion Fiscal income, per capita disposable income, government health expenditure, per capita consumption expenditurey and per capita GDP are the main economic determinants of basic medical insurance coverage for urban and rural residents.

187 Analysis on the allocation of health human resources in rural areas of China from the equity perspective

Objective To analyze the equity of rural health human resources allocation since new healthcare reform during 2010-2020. Methods The allocation of rural health human resources was described based on the data of health statistical yearbook in past years. The absolute difference, relative difference, and equity change of rural health human resource allocation were analyzed by using range method, proportion method and concentration index according to the stratification of urban and rural areas, east and west, and economic status. Results From 2010 to 2020, the number of human health resources per 1 000 rural population increased year by year. The number of health human resources per thousand population in rural areas was less than that in urban areas, and the absolute difference increases while the relative difference decreases. The absolute difference and relative difference in the number of practicing (assistant) physicians per thousand population in the eastern and western regions showed a fluctuating upward trend, and the absolute difference and relative difference in the number of registered nurses per thousand population showed a fluctuating downward trend. The difference of human resources of health per thousand population fluctuated and decreased among regions with different economic status. The concentration index of practicing (assistant) physicians per thousand population, registered nurses per thousand populations and registered nurses per square kilometer all showed a downward trend, while the concentration index of practicing (assistant) physicians per square kilometer showed a rising state in the fluctuation. Conclusion Since the new medical reform, the construction of rural health personnel has been strengthened to a certain extent, but it still falls short of the target set by the state. It is necessary to continue to expand the team and narrow the gap between urban and rural areas by attracting external talents and training internal talents. Focus on the allocation of practicing (assistant) physicians in central and western regions; consider the fairness of population distribution and geographical distribution.

European and American countries have introduced biosafety strategies to incorporate biosafety into the national governance system. China has also attached great importance to biosafety, and with the implementation of the biosafety law, China has entered a new stage of governance by the rule of law. Based on the analysis of the key points of the development of biosafety strategies in the United States, Britain, Japan, and Russia, and in combination with the construction of China’s biosafety governance system, China should further strengthen the overall national biosafety strategy, increase the investment in biosafety science and technology, pay attention to the improvement of people’s biosafety basic literacy, form a comprehensive and multi-level support of strategy-technology, state-people, and further establish and improve the national biosafety risk prevention and control and governance system.
