4 Effect and mechanism of Celastrol in the treatment of renal carcinoma based on network pharmacology and experimental verification

Objective To investigate the effect and mechanism of Celastrol (CeT) in the treatment of renal carcinoma by network pharmacology, molecular docking techniques, and experimental verification methods. Methods The key targets and pathways of CeT in the treatment of renal carcinoma were analyzed by Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform, PubChem, NCBI Gene, STRING databases, GO function, and KEGG pathway enrichment, and then Autodock Vina software was used for molecular docking verification. Human renal clear cell adenocarcinoma cell line 786-O cells were selected and divided into control group (normal culture), and 0.2, 0.4, 0.8 ¦Ìmol/L CeT group (given the corresponding concentration of CeT). After 24 h culture, apoptotic proteins and related pathway proteins in each group were detected by Western blot. Results A total of 53 CeT targets and 8 881 disease targets were screened, and 45 common targets were obtained by mutual mapping. Core targets such as Bax and Bcl-2 were obtained by analyzing common targets of protein-protein interaction network. GO enrichment analysis obtained 469 items, KEGG pathway enrichment analysis identified 67 related pathways, including apoptosis, phosp- ha tidylinositol 3 kinase (PI3K) /Akt kinase (Akt) pathway, cysteine aspartic acid specific protease-9 (caspase-9), nuclear factor ¦ÊB (NF-¦ÊB), P53, and other signaling pathways. Molecular docking results showed that CeT had good binding activity with core targets. Cell experiment results showed that the expressions of caspase-9 and P53 in 0.4 and 0.8 ¦Ìmol/L CeT groups were higher than those in the control group, while the expressions of Bcl-2 and NF-¦ÊB were lower than those in the control group, the expression of Bax in 0.4 ¦Ìmol/L CeT group was higher than that in control group, expression of PI3K in 0.8 ¦Ìmol/L CeT group was lower than that in control group, the expressions of P-Akt /Akt in 0.2, 0.4, and 0.8 ¦Ìmol/L CeT groups were lower than those in control group, and the differences were statistically significant (P£¼0.05). The expression of NF-¦ÊB in 0.8 ¦Ìmol/L CeT group was lower than that in 0.2 ¦Ìmol/L CeT group, and the differences were statistically significant (P£¼0.05). Conclusion CeT may affect the expression of target proteins such as Bax and Bcl-2 through PI3K/Akt pathway, NF-¦ÊB, and P53 signaling pathways, and play a role of multi-target and multi-pathway treatment for renal carcinoma.

11 Analysis of Professor Cai Gan’s plaster drug law based on traditional Chinese medicine inheritance computing platform

Objective To explore the application rules and characteristics of Professor Cai Gan’s ointment using traditional Chinese medicine inheritance computing platform. Methods Outpatient prescriptions of paste prescription of Professor Cai Gan from the Department of Spleen and Stomach Diseases, Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from November 2017 to December 2021 were collected. The traditional Chinese medicine inheritance computing platform V 3.0 was adopted to explore the rules of the tutor’s paste prescription, and the discrimination and treatment experience. Results A total of 1 316 plaster prescriptions were sorted out and analyzed. A total of 369 kinds of drugs were used, including 12 kinds of high-frequency drugs (frequency ≥1 000 times). The drugs of warm nature were the most commonly used drugs, followed by drugs of flat and cold nature; the drugs mainly tasted sweet, bitter, and pungent; and most drugs entered the spleen, stomach, liver, and kidney meridian; the efficacy of drugs were mainly tonify deficiency, with clearing heat, regulating qi, promoting blood, etc. According to the network topology analysis of association rules, three groups of core plaster prescriptions combinations were obtained, and five groups of characteristic drug combinations targeting accompanied syndromes were obtained by K-Means clustering. Conclusion The characteristics of professor Cai Gan’s ointment prescription are as follows: distinguishing and treating the spleen and stomach to soothe the five viscera, paying attention to the appropriate use of cold and temperature, being peaceful in nature, harmonizing the rise and fall, combining static and dynamic, and giving consideration to the drainage and complement.

15 Study on the effect of Ferula Sinkiang root extract on the abnormal proliferation of keratinocytes induced by interleukin-22

Objective To investigate the effects of different Ferula Sinkiang root extract on the abnormal proliferation of HaCaT cells, induced by interleukin-22(IL-22) and to provide evidence for evaluating the mechanism of action of Radix Aprofunda extract on HaCaT cells in psoriasis. Methods Experimental group: blank control group, IL-22 experimental group, ferulic acid low-dose, medium-dose and high-dose groups (0.5, 1, 2 mmol/L), sesquiterpenoid low-dose, medium-dose and high-dose groups (5, 10, 15 mg/ml), except blank control group, after 24 h intervention with 100 ng/ml IL-22, the cells were cultured for 24 h by adding 100 μl of different concentrations to observe cell morphology. Cell proliferation was detected by CCK8 assay. The cell cycle of each group was detected by flow cytometry. The contents of tumor necrosis factor-α(TNF-α),interferon-γ(IFN-γ), IL-23 and IL-4 in the supernatant of cultured cells were determined by enzyme-linked immunosorbent assay. Results Ferulic acid had no significant effect on the growth state of HaCaT cells, but sesquiterpenoids could inhibit the excessive proliferation of Hacat cells. There was no significant difference in cell proliferation rate between ferulic acid dose groups and IL-22 group (P>0.05). The proliferation rate of HaCaT cells in sesquiterpene dose groups was significantly different compared with that that in IL-22 experimental group (P<0.05). There was no significant difference in cell cycle between ferulic acid groups and IL-22 experimental group (P> 0.05). The percentages of G0/G1 phase, and G2/M phase in different sesquiterpenoid dose groups were significantly different compared with that those in IL-22 experimental groups (P<0.05). The cytokine levels of IL-22 experimental group were significantly different compared with that those in blank control group (P<0.05). The content of TNF-α in ferulic acid and sesquiterpene groups was lower than that in IL-22 experimental group, and the difference was statistically significant (P<0.05), but there was no significant difference between ferulic acid groups (P>0.05), and the content of TNF-α in high-dose sesquiterpene group was lower than that in low-dose sesquiterpene dose group (P<0.05). The IFN-γ content in ferulic acid and sesquiterpene groups was significantly different compared with that that in IL-22 group (P<0.05). The IFN-γ content in ferulic acid and sesquiterpene high-dose groups was lower than that in medium-dose and low-dose groups (P<0.05). The IFN-γ content in medium-dose group was lower than that in low-dose group (P<0.05). The content of IL-23 in sesquiterpenoid dose groups were significantly different compared with that in the IL-22 experimental group (P<0.05). The content of IL-23 in sesquiterpenoid high-dose group was lower than that in sesquiterpenoid medium-dose group and low-dose group (P<0.05). There was no significant difference in IL-4 content among treatment groups (P>0.05). Conclusion Sesquiterpenoids can inhibit IL-22-induced abnormal proliferation of HaCaT cells and regulate cell cycle, which is the important component in the treatment of psoriasis.

20 Expression and clinical significance of regulator of G-protein signaling 10 in renal clear cell carcinoma

Objective To investigate the expression and clinical significance of regulator of G-protein signaling 10 (RGS10) gene in renal clear cell carcinoma. Methods The transcriptome data of renal clear cell carcinoma were downloaded from the cancer genome atlas database. The differential expression and clinical significance of RGS10 gene in renal clear cell carcinoma was analyzed. Using the median RGS10 gene expression as the dividing line, 266 cases were divided into low expression group (RGS10≤8.776) and 266 cases were divided into high expression group (RGS10 > 8.776). Kaplan-Meier method was used to analyze the relationship between RGS10 expression and survival prognosis in patients with renal clear cell carcinoma. Multivariate Cox regression was used to analyze the prognostic factors of patients with renal clear cell carcinoma. The receiver operating characteristic(ROC) curve was used to analyze the predictive value, and the relevant signaling pathways regulated by RGS10 were identified by gene enrichment analysis. Results The expression of RGS10 gene in renal clear cell carcinoma tissues was higher than that in adjacent tissues (P<0.001). ROC curve results showed that the area under the curve of RGS10 expression in predicting renal clear cell carcinoma was 0.920, 95%CI was 0.896-0.940, the specificity was 90.3%, and the sensitivity was 85.4% (P<0.001). The expression of RGS10 was correlated with gender, pathological grade, T stage, N stage, M stage, and clinical stage of patients with renal clear cell carcinoma (P<0.01). Kaplan-Meier survival curve suggested that the overall survival prognosis of the high expression group was worse than that of the low expression group (P< 0.001). Univariate analysis showed that age, tumor location, pathological grade, clinical stage, T stage, N stage, M stage,  and RGS10 expression level were correlated with the survival of patients with renal clear cell carcinoma (P<0.05). Multivariate Cox regression analysis showed that patients’ age (HR=1.756, 95%CI: 1.148-2.686), M stage (HR=2.727, 95%CI: 1.595-4.660) and RGS10 expression level (HR=1.834, 95%CI: 1.148-2.930) were independent prognostic factors of patients with renal clear cell carcinoma (P<0.05). Gene enrichment analysis showed that the function of RGS10 was mainly concentrated in Toll-like receptor signaling pathway, FcγR-mediated phagocytosis signaling pathway, and B cell receptor signaling pathway. Conclusion RGS10 is a potential diagnostic and prognostic molecular marker for renal clear cell carcinoma. In renal clear cell carcinoma, Toll-like receptor signaling pathway, FcγR-mediated phagocytosis signaling pathway, and B cell receptor signaling pathway may be regulated by RGS10 gene.

25 PIK3CA gene mutation in gynecological tumors and its relationship with median survival time of patients

Objective To investigate PIK3CA gene mutation in gynecological tumors and to analyze its relationship with the survival time of patients. Methods The tumor tissue samples of 110 patients with gynecological tumors admitted to the First Affiliated Hospital of Guangdong Pharmaceutical University from January 2017 to January 2021 were selected, including 30 cases of endometrial cancer, 40 cases of cervical cancer, and 40 cases of ovarian cancer. PIK3CA gene mutation was detected in 110 patients with gynecological tumors, and the relationship between PIK3CA gene mutation and prognosis was analyzed. Results Among 110 patients, 61 cases (55.45%) had PIK3CA gene mutation, and the most common base mutation was c.G1633A. The mutation rates of PIK3CA in cervical cancer and ovarian cancer were 57.5% and 67.5%, respectively. There was statistical significance in the incidence of PIK3CA gene mutation between cancer tissues and adjacent tissues in patients with cervical cancer and ovarian cancer (P<0.05). The survival time of patients with PIK3CA mutation type in cervical cancer and ovarian cancer was shorter than that of PIK3CA wild type, and the difference was highly statistically significant (P<0.01); there was no significant difference between PIK3CA mutant type and wild type in endometrial cancer (P > 0.05). Conclusion PIk3CA gene mutation exists in gynecological tumors, especially in ovarian cancer and cervical cancer, and is closely related to the survival time of patients.

29 Construction of a line graph model for predicting the risk of acute respira- tory distress syndrome after valve replacement under extracorporeal circulation

Objective To investigate the risk factors of acute respiratory distress syndrome after valve replacement under extracorporeal circulation and to construct a columnar graphical model of the risk of its occurrence. Methods Two hundred and eleven patients who underwent valve replacement surgery under extracorporeal circulation at the Affiliated Hospital of Chengde Medical College from November 2016 to December 2020 were selected and divided into 16 cases in the experimental group and 195 cases in the control group based on whether acute respiratory distress syndrome occurred after surgery. Multi-factor logistic regression analysis was used to obtain their independent risk factors; R software was applied to develop column line graph models and validate their predictive efficacy. Results The differences in age, body mass index, cerebral infarction, cardiac function class, replacement of biological valves, time to extracorporeal circulation, time to aortic block, and second open heart were statistically significant when comparing the two groups (P<0.05). The logistic regression results showed that age (OR[95%CI]=1.266[1.057, 1.517]), body mass index (OR[95%CI]=1.860[1.158, 2.988]), and duration of extracorporeal circulation (OR[95%CI]=1.089[1.025, 1.158]) were independent risk factors for the development of ARDS (P<0.05). The column line graph model was constructed based on the above independent risk factors, and external validation showed that the consistency index of the model was 0.918 and the area under the curve was 0.922, which had good consistency and predictive efficacy. Conclusion To construct a nomogram of ARDS after valve replacement under CPB, which can realize visualization and individualized prediction, has guiding significance for formulating individualized preventive measures, and has high clinical value.

34 Application of functional near-infrared spectroscopy in the study of brain  neurocognitive disorders

In this paper, the principles, advantages and disadvantages, current situation and prospects of near infrared spectroscopy in the study of neurocognitive disorders in the brain are reviewed. The principle is to use the optical properties of near infrared spectrum to measure the neuronal activity of the brain, so as to determine the location of brain neurocognitive disorders. It has the advantages of portability, noise-free and high ecological validity. The main research site is located in the prefrontal cortex and the whole brain. The results show that the results of functional near-infrared spectrum signals in the study of neurocognitive disorders in the brain are consistent with the results of traditional neuroimaging techniques (such as functional magnetic resonance imaging), namely, age has a negative effect on brain function in general, and neurocognitive disorders occur mainly in the prefrontal cortex. At present, there are still some shortcomings in the research of functional near infrared spectroscopy. It can be combined with other technologies to improve the temporal and spatial resolution, so as to study neurocognitive disorders of the brain in a more systematic, comprehensive and accurate way.

38 Research progress on the role of candida mycoderma bacteria in female vaginal microecosystem

Candida mycoderma bacteria is an opportunistic pathogen, which is one of the main causes of vulvovaginitis. With the wide use of genomics tools, the important and complex role of vaginal fungal community in human health and disease are increasingly recognized. Exploring the fungal species, functions and interactions with different microbial species in vaginal microecosystem is very important for the treatment and prevention of vaginitis and cervical diseases, and the improvement of pregnancy outcomes and reproductive health. This paper reviews the role of candida mycoderma bacteria in female health and disease.

43 Correlation between autophagy and Candida pathogenicity and drug resistance

Autophagy is an inherent degradation metabolic pathway of eukaryotic cells. It is involved in biological processes such as organelle degradation, nutrient balance, and programmed death to regulate the morphological transformation and cell differentiation required for the key links of fungal infection. Moderate autophagy protects fungi against a poor living environment, and an imbalance of autophagy leads to autophagic cell death. Candida is the leading pathogen causing invasive fungal infections in hospitalized patients. Autophagy plays an important role in regulating the pathogenicity and drug resistance of Candida. Therefore, this paper aims to review the molecular mechanism of autophagy and the research status of autophagy in Candida, in order to provide new ideas for the prevention-treatment strategies of Candida and the research and development of antifungal drugs.

47 Research progress of acupuncture in the treatment of idiopathic facial palsy

Idiopathic facial paralysis (IFP) is a facial nerve dysfunction induces by acute non-specific inflammation. Studies have shown that acupuncture can promote the recovery of facial nerve functions, and alleviate complications by controlling inflammation and edema, improving circulation, regulating immunity, etc. Although some consensus has been reached on the IFP treatment by acupuncture, there are still disputes on the optimal intervention timing, the acupuncture manipulation, and acupoint selection. This paper summarizes recent studies and finds that early intervention should be followed in IFP acupuncture treatment. Acupoint selection should be combined with syndrome differentiation and meridians differentiation, local and distal, and emphasize the combination of disease stages to select acupoints, and adjust acupuncture techniques. Different acupuncture treatment methods and traditional Chinese medicine external treatment can be used in combination, complementary advantages, improve the effect.

52 Research progress on exercise intervention and mechanism in college student subthreshold depression

The prevalence of subthreshold depression in college students is much higher than that in the general population, which is easy to be ignored and has serious consequences. Exercise intervention, as an important non-pharmacological treatment, has a good effect on the regulation of depression and other neurological and psychiatric diseases. However, there are few systematic reports on exercise intervention for subthreshold depression in college students. Therefore, in order to better prevent and treat subthreshold depression in college students, this paper makes a comprehensive and systematic collation and review of the current situation and diagnostic tools of subthreshold depression in college students, and the effects and potential biological mechanisms of different exercise paradigms in the intervention of subthreshold depression in college students are analyzed and summarized in order to provide new ideas and theoretical support for further exploration of the prevention and treatment of subthreshold depression in college students.

56 Research progress on the mechanism of circular RNA in virus replication

Virus replication is the key node for virus to survive and reproduce in the host, which is very harmful to human health. Humoral immunity and cellular immunity are the main means for the body to regulate virus replication. However, due to the complexity and diversity of the human immune system, most of the mechanisms of virus replication are still unclear. Circular RNA is a kind of non coding RNA with the function of gene expression regulation. It regulates viral replication by regulating immune response factors, miRNA, proteins, replication starting points, DNA synthesis and assembly, and other processes. Although there are problems such as low expression of circular RNA and high cost of sequencing and library building, circular RNA is still a good method to explain the mechanism of virus replication.

60 Analysis of influencing factors of susceptibility to stress insomnia in freshmen of medical school

Objective To explore the influencing factors of susceptibility to stress insomnia in freshmen of medical school. Methods In September 2021, the cluster sampling method was used to conduct a questionnaire survey among 663 freshmen from the Medical College of Hefei Vocational and Technical College, Anhui Province. According to the Chinese version of Ford insomnia response to stress test Chinese version (FIRST-C), the susceptibility to stress insomnia was assessed and the students were divided into high risk group (313 cases) and low risk group (350 cases). The general information questionnaire, insomnia severity index (ISI) scale, 7-item generalized anxiety disorder (GAD-7) scale, patient health questionnaire (PHQ-9) and Connor-Davidson resilience scale (CD-RISC) were compared between the two groups, and the influencing factors of susceptibility to stress insomnia among freshmen in the medical school were analyzed. Results There were significant differences in relationship and childhood abuse of parents between the two groups (P< 0.05). The scores of ISI, GAD-7, and PHQ-9 in high risk group were higher than those in low risk group, and the total scores of CD-RISC and all factors were lower than those in low risk group, and the differences were statistically significant (P< 0.05). The severity of insomnia in high-risk group was higher than that in low-risk group, and the detection rate of anxiety and depression was higher than that in low-risk group, and the differences were statistically significant (P<0.05). Logistic regression results showed that GAD-7 and PHQ-9 scores were risk factors for the susceptibility of freshmen to stress insomnia (OR>1, P<0.05), and total score of CD-RISC were protective factors for the susceptibility of freshmen to stress insomnia (OR<1, P> 0.05). Conclusion Anxiety, depression and mental resilience are the factors affecting the susceptibility of new medical students to stress insomnia.

65 Analysis of adverse reactions of 132 children caused by Yanhuning for injection in Hunan Children’s Hospital from 2006 to 2020

Objective To summarize and analyze the occurrence of adverse drug reactions (ADR) caused by children with Yanhuning for injection in Hunan Children’s Hospital(“our hospital” for short), and to promote the safe and rational use of drugs in children’s clinics. Methods A retrospective analysis method was used to statistically analyze 132 reports of ADR that occurred after treatment with Yanhuning for injection in our hospital from January 2006 to December 2020, as well as to analyze the root causes of ADR in combination with fishbone diagrams and to evaluate the rationality of drug use. Results ADR of 132 cases caused by  Yanhuning for injection occurred mainly in infants and children aged 0-3 years; the most important clinical manifestations were skin and adnexal damage; 12 cases (9.09%) were severe ADR. the occurrence of ADR was related to super-indication, super-therapeutic dosing, intravenous drug concentration and drip rate, and patients’ own factors. Conclusion The clinic should strengthen the clinical rational use of Yanhunin for injection, timely monitoring and intervention to reduce the occurrence of adverse reactions and improve the safety of drug use in children.

Objective To evaluate the effect of computer game cognitive function training combined with Butylphthalide soft Capsules in the elderly with vascular mild cognitive impairment. Methods From January 2019 to December 2020, a total of 66 elderly patients with vascular mild cognitive impairment were enrolled in outpatients from four hospitals (Liaoning Jinqiu Hospital, Liaoning Provincial People’s Hospital, the First Affiliated Hospital of China Medical University and Shengjing Hospital) in Shenyang, and demographic and clinical data were recorded. According to the random number table, they were divided into observation group and control group. The observation group was given computer adaptive gaming cognitive training combined with Butylphthalide Soft Capsules, while the control group was given single drug treatment with Butylphthalide soft capsules, with 33 cases in each group. Scores of overall cognitive function (MoCA), number span test, auditory vocabulary learning test (AVLT), connection test (STTA-STTB) and Stroop color word test were compared between the two groups before intervention, immediately after intervention and one month after intervention. Results There were significant differences of the anteriorly and backward scores of digital span test in the time, intergroup, and interaction between the two groups (P<0.05). Further pair comparison showed that immediately after intervention, the anteriorly and backward scores of digital span test in the two groups were higher than before intervention, while those of the observation group were higher than those of the control group; the number span test score of the observation group was higher than that of the control group one month after intervention, and the differences were statistically significant (P<0.05). There were statistically significant differences of AVLT recognition in scoring of the anteriorly and backward scores of digital span test time, inter-group and interaction between the two groups (P<0.05). The total time of AVLF immediate memory between the two groups was statistically significant (P<0.05). Further pairwise comparison showed that the total scores of AVLF recognition and immediate memory in the two groups were higher immediately after intervention than before intervention; one month after the intervention, the AVLT scores of the observation group were higher than those before the intervention, and the differences were statistically significant (P<0.05). The comparison of STT-A between the two groups was statistically significant (P<0.05). Comparison of STT-B in time, intergroup, and interaction between the two groups showed statistically significant differences (P<0.05). Further pair-to-pair comparison showed that STT-A in the observation group was lower than that in the control group at the same time point immediately after intervention and a month after intervention; immediately after intervention, STT-B in both groups was lower than before intervention, and the observation group was lower than the control group; one month after the intervention, STT-B in the observation group was lower than before intervention, and lower than that in the control group, the differences were statistically significant (P<0.05). The  comparison of  StroopA in time and interaction between the two groups were statistically significant (P<0.05). The  comprison of StroopB in time, intergroup and interaction between the two groups were statistically significant (P<0.05). Further pair comparison showed: immediately after intervention, StroopA in both groups was lower than before intervention; one month after intervention, StroopA in observation group was lower than before intervention, and lower than control group, the differences were statistically significant (P<0.05). Immediately after intervention, StroopB in the two groups was lower than before intervention, and the observation group was lower than the control group; one month after intervention, StroopA in the observation group was lower than before intervention and lower than the control group, with statistical significance (P<0.05). There were statistically significant differences of total MoCA in time, intergroup, and interaction between the two groups (P<0.05). Further pair comparison showed that the total score of MoCA of cognitive function in the observation group was higher than that before intervention and was higher than that in the control group after intervention, and the differences were statistically significant (P<0.05). Conclusion The application of computer game cognitive function training combined with Butylphthalide Soft Capsules therapy in the elderly with vascular mild cognitive impairment has a significant effect, which can effectively improve the cognitive dysfunction and improve the quality of life of patients.

Objective To explore the effect of different doses of Topiramate combined with Olanzapine in the treatment of schizophrenia and its effect on body mass index (BMI) and glucose and lipid metabolism. Methods A total of 120 patients with schizophrenia who underwent outpatient and inpatient treatment in the Department of Psychiatry, the Huzhou Third People’s Hospital from November 2020 to February 2022 were selected and they were divided into control group, experimental group 1, and experimental group 2 according to the random number table method, with 40 patients in each group. The control group was treated with Olanzapine, the experimental group 1 was treated with Topiramate (maximum dose of 100 mg) + Olanzapine, and the experimental group 2 was treated with Topiramate (maximum dose of 200 mg) + Olanzapine for 12 weeks. The scores of positive and negative syndrome scale (PANSS), BMI, and glucose and lipid metabolism indexes (fasting blood glucose [FBG], fasting insulin [FINS], triglyceride [TG], low-density lipoprotein cholesterol [LDL-C], total cholesterol [TC] levels, and safety were compared among the three groups. Results Before treatment, there was no significant difference in PANSS scores among the three groups (P>0.05). Comparison between experiment group 1 and control group: there was no significant difference in PANSS score between experimental group 1 and control group at 4 weeks (P>0.05), the PANSS score of experiment group 1 was lower than that of control group after 8 and 12 weeks of treatment, and the differences were statistically significant (P<0.05). Comparison between experimental group 2 and control group: after 4, 8 and 12 weeks of treatment, PANSS score of experimental group 2 was lower than those of control group, and the differences were statistically significant (P<0.05). Comparison between experiment group 1 and experiment group 2: the PANSS score of experiment group 2 were lower than that of experiment group 1 after 4, 8 and 12 weeks of treatment, and the differences were statistically significant (P<0.05). Intra-group comparison among the three groups: compared with before treatment, PANSS scores in the three groups decreased after 4, 8 and 12 weeks of treatment, and the differences were statistically significant (P<0.05). After treatment, the levels of BMI, FBG, FINS, TG, LDL-C, and TC in experimental group 1 and experimental group 2 were lower than those in control group (P<0.05), but there was no statistical significance between experimental group 1 and experimental group 2 (P>0.05). There was no significant difference in the total incidence of adverse reactions among the three groups (P>0.05). There was significant difference in the incidence of paresthesia among the three groups (P<0.05). Conclusion The treatment of Topiramate in patients with schizophrenia can improve the body weight increase caused by olanzapine, regulate the metabolism of glucose and lipids, and relieve mental symptoms; the 200 mg dose of topiramate has a more obvious improvement effect, but it can increase the incidence of paresthesias.

81 Effect of low frequency repetitive transcranial magnetic stimulation on lower limb spasticity and balance gait in patients with hemiplegia after stroke

Objective To investigate the effect of low frequency repetitive transcranial magnetic stimulation on lower limb spasticity and balanced gait in patients with hemiplegia after stroke. Methods A total of 176 patients with hemiplegia after stroke admitted to the First Affiliated Hospital of China Medical University from April 2021 to April 2022 were selected and divided into control group and observation group by random number table method, with 88 cases in each group. The control group was treated with routine rehabilitation + core strength training, and the observation group was additionally treated with low frequency repetitive transcranial magnetic stimulation. Two weeks was a course of treatment, and both groups were treated for three consecutive courses. The clinical efficacy were compared, and the symptom score and balance gait parameters before and after treatment were compared between the two groups, and the safety of treatment was evaluated. Results The clinical effect of observation group was better than control group (P<0.05). After treatment, clinical spasm index (CSI) scores of both groups were lower than those before treatment, Berg balance scale (BBS) and lower limb Fugl-Meyer motor function assessment-lower limb evaluation (FMA-LE) scores were higher than those before treatment, and CSI of observation group were lower than control group, BBS and FMA-LE scores were higher than control group (P<0.05). After treatment, the peak value of hip flexion, peak value of knee flexion, stride length, stride frequency, and stride speed in both groups were higher than before treatment, and the observation group was higher than the control group (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups during treatment (P>0.05). Conclusion Low frequency repetitive transcranial magnetic stimulation is effective in the treatment of lower limb spasticity in patients with hemiplegia after stroke, and can improve lower limb function, balance function, and walking function, with good safety.

Objective To investigate the association between monocyte chemoattractant protein-1 (MCP-1), interleukin-6 (IL-6), nitric oxide (NO), and cognitive function in patients with cerebrovascular disease (CSVD). Methods A total of 130 patients diagnosed with CSVD in the First Affiliated Hospital of Hebei North University (hereinafter referred to as “our hospital”) from September 2020 to September 2021 with Montreal cognitive assessment (MoCA) score <26 points were collected as the observation group. In addition, 80 patients with no abnormal brain imaging and MoCA score ≥26 points in our hospital during the same period were collected as the control group. Serum MCP-1, IL-6, and NO were compared between the two groups, and the influencing factors of CSVD cognitive dysfunction were analyzed. The diagnostic value was analyzed by the receiver operator characteristic curve, and the correlation between MCP-1, IL-6, NO, and the MoCA score of patients was further analyzed. Results The levels of serum MCP-1 and IL-6 in the observation group were higher than those in the control group, and the serum NO and MoCA scores were lower than those in the control group, the differences were statistically significant (P<0.05). Multivariate analysis showed that serum MCP-1 (OR=1.055, 95%CI: 1.037-1.074), IL-6 (OR=1.659, 95%CI: 1.431-1.923), NO(OR=0.836, 95%CI: 0.785-0.891) level was independent factors of CSVD cognitive dysfunction. The results of receiver operator characteristic curve analysis showed that the area under the curve of serum MCP-1, IL-6, and NO combined to predict CSVD cognitive dysfunction was 0.880 (P<0.05), the sensitivity was 82.3%, and the specificity was 83.8%. Serum NO level was positively correlated with MoCA scores in patients with CSVD cognitive dysfunction (r>0, P<0.05), while serum MCP-1 and IL-6 levels were negatively correlated with it (r<0, P<0.05). Conclusion In patients with CSVD cognitive dysfunction, the levels of serum MCP-1 and IL-6 are increased, while the level of NO is decreased, which are correlated with the level of cognitive function. The combined detection of the three indicators is helpful to predict the occurrence of CSVD cognitive dysfunction.

90 Clinical value of glycosylated hemoglobin in type 2 diabetes mellitus complicated with post-stroke pneumonia

Objective To explore the clinical value of glycosylated hemoglobin (HbA1c) in type 2 diabetes mellitus (T2DM) complicated with post-stroke pneumonia. Methods A total of 120 patients with T2DM complicated with post-stroke pneumonia admitted to Rugao People’s Hospital (hereinafter referred to as “our hospital”) of Jiangsu Province from December 2019 to December 2021 were selected as the study group, and 112 patients with T2DM complicated with stroke and no pneumonia admitted to our hospital during the same period were selected as the control group. The clinical features of the two groups were compared, and the factors affecting the incidence of T2DM complicated with post-stroke pneumonia were analyzed, the value of serum HbA1c level in predicting the incidence of T2DM complicated with post-stroke pneumonia was further analyzed by receiver operating characteristic (ROC) curve. Results There were significant differences in the proportion of consciousness disorder, National Institutes of Health stroke scale score, C-reactive protein, procalcitonin, fasting blood glucose, white blood cell count, and HbA1c level between the study group and the control group (P<0.05). The results of multivariate analysis showed that HbA1c level (OR=3.177, 95%CI: 1.143-8.828) and disorder of consciousness (OR=2.971, 95%CI: 1.587-5.564) were independent factors for the occurrence of T2DM complicated with post-stroke pneumonia (P<0.05). ROC curve results showed that the cut-off value was 7.83%, sensitivity was 84.17%, specificity was 95.54%, and area under the curve was 0.930 for serum HbA1c level to predict T2DM complicated with post-stroke pneumonia (P<0.05). Conclusion Serum HbA1c level is abnormally elevated in patients with T2DM complicated with post-stroke pneumonia, and HbA1c level is an independent influencing factor of T2DM complicated post-stroke pneumonia. Dynamic monitoring of Hba1c level is of great value in predicting the occurrence of T2DM complicated with post-stroke pneumonia.

94 Effects of low molecular weight heparin combined with Adenosine Triphosphate and Coenzyme A on fetal growth restriction

Objective To observe the effects of low molecular weight heparin combined with Adenosine Triphosphate and Coenzyme A on fetal growth restriction. Methods A total of 240 pregnant women with fetal growth restriction admitted to Beijing Huairou Hospital from January 2017 to December 2020 were selected as study object, and they were divided into control group (120 cases, receiving Adenosine Triphosphate combined with Coenzyme A) and study group (120 cases, receiving low molecular weight heparin on the basis of control group) by random number table method. The treatment time of both groups was one week. Fetal growth and development, clinical efficacy, gestational age, neonatal umbilical cord blood gas, and maternal and infant outcomes were compared between two groups; amniotic fluid indexes before and after treatment were compared between two groups. Results Fetal head circumference, abdominal circumference, double parietal diameter, and uterine height in the study group were higher than those in the control group (P<0.05). The clinical effect of the study group was better than that of the control group (P<0.05). After treatment, the amniotic fluid indexes in both groups were higher than those before treatment, and those in the study group were higher than those in the control group (P<0.05). The gestational week and oxygen partial pressure of study group were higher than those of control group, while the carbon dioxide fraction was lower than that of control group (P<0.05); there was no significant difference in delivery mode between the two groups (P>0.05). The total incidence of pregnancy complications in study group was lower than that in control group (P<0.05). The incidence of preterm infants and full-term infants in study group was lower than that in control group, and Apgar score was higher than that in control group (P<0.05); there were no significant differences in neonatal asphyxia rate and perinatal mortality between two groups (P>0.05). Conclusion Low molecular weight heparin combined with Adenosine Triphosphate and Coenzyme A is effective in the treatment of fetal growth restriction, can promote fetal growth and development, and can improve maternal and infant outcomes.

Objective To explore the differential diagnostic value of fractional exhaled nitric oxide (FeNO) combined with small airway function tests in children with chronic cough caused by cough variant asthma (CVA) and post-infectious cough. Methods A total of 89 children with chronic cough who were admitted to the Affiliated Hospital of Yangzhou University from May 2018 to July 2021 were selected as study object, they were divided into CVA group (34 cases) and post-infection cough group (55 cases) according to different etiologies. The FeNO concentration and small airway function (forced expiratory volume in the first second [FEV1], forced expiratory flow at 50% vital capacity [FEF50], forced vital capacity [FVC], maximum mid-expiratory flow 75/25 [MMEF75/25]) of two groups were compared on the 1st day of admission. The influencing factors of CVA-induced chronic cough were analyzed. Results There were no significant difference in age, sex, cough time, lymphocyte fraction, neutrophil fraction, daytime cough symptom score, and nighttime cough symptom score between two groups (P>0.05). FeNO concentration in CVA group was higher than that in post-infection cough group, FEF50 and MMEF75/25 were lower than those in post-infection cough group, and the differences were statistically significant (P<0.05); there were no significant differences in FEV1 and FVC between two groups (P>0.05). FeNO concentration, MMEF75/25, and FEF50 were the influencing factors of CVA-induced chronic cough (OR>1, P<0.05). The area under the curve of FeNO combined with FEF50 and MMEF75/25 was 0.894, the sensitivity was 88.24%, and the specificity was 87.27%. Conclusion FeNO, MMEF75/25,and FEF50 have certain value in the differential diagnosis of CVA-induced chronic cough.

102 Diagnosis value of serum leukocyte immunoglobulin-like receptor subfamily B2 level in the diagnosis of colorectal cancer

Objective To investigate the diagnosis value of leukocyte immunoglobulin-like receptor subfamily B2 (LILRB2) levels in patients with colorectal cancer (CRC). Methods A total of 125 CRC patients diagnosed in Affiliated Hospital of Weifang Medical University (“our hospital” for short) from January 2021 to May 2022 were selected as CRC group, and 120 patients who underwent physical examination in our hospital during the same period were selected as control group. The serum LILRB2, carcinoembryonic antigen (CEA), and carbohydrate antigen 199 (CA199) levels of the CRC group and control group were detected using enzyme-linked immunosorbent assay and electrochemiluminescence method, respectively. Data were statistically processed to analyze the diagnostic value of LILRB2 in patients with CRC. Results Serum LILRB2, CA199, and CEA expressions in the CRC group were higher than those in the control group, the differences were statistically significant (P<0.05). Statistically significant differences were observed in serum LILRB2 levels among patients of different ages, lymph node metastasis, vascular tumor thrombus, depths of invasions, and TNM stages (P<0.05). Receiver operating characteristic curve results showed, the area under curve for serum LILRB2, CA199, CEA, and combined detection of the three were 0.7593, 0.5727, 0.7234, and 0.7889, respectively. The diagnostic efficiency of the combined detection of the three was higher than that of CA199 and CEA alone, the differences were statistically significant(P<0.05). There were no significant differences between the combined detection of the three and LILRB2 (P>0.05). The diagnostic sensitivity of LILRB2 in TNM Ⅰ+Ⅱ patients was higher than that in TNM Ⅲ+Ⅳ patients, the differences were statistically significant (P<0.05). Conclusion Serum LILRB2 level in CRC patients has certain clinical value in diagnosis of CRC.

Objective To re-evaluate the quality of published systematic reviews of Pembrolizumab for advanced non-small cell lung cancer. Methods Systematic reviews of Pembrolizumab for advanced non-small cell lung cancer were collected from PubMed, EMBase, Cochrane Library, Web of Science, CNKI, VIP, and Wanfang databases from to February 2022. PRISMA statement, AMSTAR 2 and GRADE tools were used to evaluate the reporting quality, methodological quality, and evidence quality of the included systematic reviews. Results A total of ten systematic reviews were included. The PRISMA score ranged from 20-26. The evaluation results of AMSTAR 2 tool indicated that eight items were reported completely, two items were not reported. The grading results of GRADE showed that the outcome indexes of systematic reviews were mainly of low quality. Conclusion Current evidence shows that Pembrolizumab has significant clinical benefits in the treatment of advanced non-small cell lung cancer. However, the methodological quality of systematic reviews is insufficient, and the evidence quality of evidence-based medicine needs to be further improved.

111 Clinical characteristics and prediction strategy of non-recurrent laryngeal nerve related to thyroid surgery

Objective To investigate the clinical characteristics and prediction strategy of non-recurrent laryngeal nerve (NRLN) related to thyroid surgery. Methods The clinical data of 175 patients who underwent thyroid surgery for benign and malignant thyroid tumors from August 2009 to February 2022 in Department of Comprehensive Surgery, Medical and Health Center, Beijing Friendship Hospital (hereinafter referred to as “our department”) were retrospectively analyzed; in addition, only enroll Chinese patients as study objectives in CNKI, Wanfang, PubMed, and Web of Science database were retrieved by using “non-recurrent laryngeal nerve”,“non recurrent laryngeal nerve”, “non-recurrent inferior laryngeal nerve”, “recurrent laryngeal nerve”, “inferior laryngeal nerve”, “NRLN”, and “NRILN” as search terms, the search period was from January 2012 to February 2022. The total incidence, the incidence of right side, Stewart classification, injury rate of NRLN, and the results of NRLN predicted by preoperative imaging were counted. Results One right NRLN, Stewart type Ⅰ, was found in our department. The total incidence of NRLN was 0.32%, and the incidence of right NRLN was 0.63%, and there was no intraoperative injury in all cases. Meta-analysis showed that the total incidence of NRLN was 0.46% (95%CI: 0.26%-0.66%); the incidence of right side was 0.45% (95%CI: 0.22%-0.68%); the incidence of Stewart type Ⅰ was 76.47% (95%CI: 58.61%-94.33%), and type Ⅱ was 23.53% (95%CI: 5.67%-41.39%); the injury rate was 10.20% (95%CI: 2.93%-17.47%). Imaging prediction showed the presence of an anomalous right subclavian artery in 90% (81/90); the rate of missed diagnosis of NRLN predicted by CT was 19.23%, the misdiagnosis rate of NRLN predicted by chest X-ray was 56.67%, the rate of missed diagnosis of NRLN predicted by ultrasound was 7.25%. Conclusion NRLN is a rare variation with high risk of intraoperative injury, and can be predicted preoperatively by imaging examination, CT and ultrasound scan demonstrate high accuracy.

118 Efficacy of Canagliflozin and Metformin in type 2 diabetes mellitus combined with mild obstructive sleep apnea syndrome

Objective To compare the effects of Canagliflozin and Metformin in type 2 diabetes mellitus (T2DM) combined with mild obstructive sleep apnea hypopnea syndrome (OSAHS). Methods Sixty patients with T2DM complicated with mild OSAHS admitted to Baise People’s Hospital of Guangxi Zhuang Autonomous Region from June 2020 to June 2021,  according to random number table method, they were divided into Canagliflozin group and Metformin group, with 30 cases in each group. Canagliflozin group received Canagliflozin orally and Metformin group received Metformin orally, all were treated for 12 weeks. Blood glucose index, blood lipid index, body mass index (BMI), blood pressure, ventilation function, and the occurrence of adverse reactions were compared between the two groups before and after treatment. Results After treatment, fasting blood glucose, 2 h postprandial blood glucose, glycosylated hemoglobin, and insulin resistance index of the two groups were lower than before treatment, the differences were statistically significant (P<0.05), but there was no statistical significance between the two groups (P>0.05). After treatment, total cholesterol (TC), triglyceride (TG), and low density lipoprotein cholesterol (LDL-C) in both groups were lower than before treatment, while high density lipoprotein cholesterol (HDL-C) was higher than before treatment, and TC, TG, and LDL-C in Canagliflozin group were lower than those in Metformin group, while HDL-C was higher than those in Metformin group. After treatment, BMI, systolic blood pressure, diastolic blood pressure, and apnea hypopnea index (AHI) in Canagliflozin group were lower than before treatment, and lowest percutaneous arterial oxygen saturation (LSpO2) was higher than before treatment, and BMI, systolic blood pressure, diastolic blood pressure, and AHI in Canagliflozin group were lower than those in Metformin group, while LSpO2 was higher than those in Metformin group, the differences were statistically significant (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05). Conclusion Compared with Metformin, Canagliflozin can not only effectively improve blood glucose and lipid levels in T2DM patients with mild OSAHS, but also effectively improve blood pressure, ventilation function and weight loss.

Objective To evaluate the efficacy of lymphocyte count (Lym), interleukin-6 (IL-6), and interleukin-10 (IL-10) combined detection in sepsis early screening. Methods A total of 153 infected patients admitted to the First Affiliated Hospital of Chongqing Medical University from January 2021 to March 2022 were selected. Patients with Sepsis-3 were classified into sepsis group (81 cases) and non-sepsis group (72 cases). Serum Lym, IL-6, and IL-10 and other indicators were collected within 2 h after the onset of sepsis, the influencing factors of the occurrence of sepsis were analyzed, the diagnostic efficacy of Lym, IL-6, and IL-10 on sepsis was analyzed by receiver operating characteristic curve. Results Lym and lymphocyte percentage (L%) in sepsis group were lower than those in non-sepsis group, and age, IL-6, IL-10, and neutrophil count/lymphocyte count ratio were higher than those in non-sepsis group, with statistical significance (P<0.05). Lym and L% were protective factors for sepsis (OR=0.141, 0.798, P<0.05), IL-6 and IL-10 were risk factors for sepsis (OR=1.007, 1.080, P<0.05). The area under the curve of Lym, IL-6, and IL-10 combined detection was higher than that of each single detection, with statistical significance (P<0.05). Conclusion Lym, IL-6, and IL-10 are closely related to sepsis, and the combined detection of sepsis is better than that of detection alone.

128 Preoperative diagnostic value of two-dimensional ultrasound combined with BRAF gene detection for benign and malignant thyroid nodules undefined by FNAC#br#

Objective To explore the preoperative diagnostic value of two-dimensional ultrasound combined with BRAF gene detection for benign and malignant thyroid nodules undefined by fine-needle aspiration cytology (FNAC). Methods A total of 119 patients with thyroid nodules admitted to Huaian First People’s Hospital from June 2019 to December 2021 were selected as the study subjects. Two-dimensional ultrasound and BRAF gene detection were performed before the operation. The pathological results after the operation were taken as the gold standard, the diagnostic value of two-dimensional ultrasound combined with BRAF gene detection for benign and malignant thyroid nodules was analyzed, Kappa test was performed for the diagnostic results. Results Pathological results showed that 87 benign thyroid nodules and 32 malignant thyroid nodules. The accuracy, sensitivity, and specificity of two-dimensional ultrasound in the diagnosis of benign and malignant thyroid nodules were 81.51%, 90.62%, and 78.16% respectively; Kappa test results showed that there was general consistency between two-dimensional ultrasound and pathological findings in the evaluation of benign and malignant thyroid nodules (Kappa=0.594, P<0.01). The accuracy, sensitivity, and specificity of BRAF gene detection in the diagnosis of benign and malignant thyroid nodules were 89.92%, 93.75%, and 88.51% respectively; Kappa test results showed that BRAF gene detection and pathologic results were generally consistent in evaluating benign and malignant thyroid nodules (Kappa=0.637, P<0.01). The accuracy, sensitivity, and specificity of two-dimensional ultrasound combined with BRAF gene detection were 83.19%, 96.86%, and 78.16%, respectively; Kappa test results showed that there was a good consistency between two-dimensional ultrasound combined with BRAF gene detection and pathological findings in the evaluation of benign and malignant thyroid nodules (Kappa=0.762, P<0.01). Conclusion Two-dimensional ultrasound combined with BRAF gene detection can improve the sensitivity of preoperative diagnosis and has important significance in preoperative diagnosis of benign and malignant thyroid nodules.

Objective To analyze the medication rule of focal adenomyopathy (AM) patients of different ages in the Affiliated Hospital of Shandong University of Traditional Chinese Medicine, so as to expand the idea of conservative and phased treatment of AM. Methods Data of focal AM patients in the outpatient department of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from January 2012 to December 2017 were collected. Frequency analysis, multi-scale backbone network analysis, and other data mining methods were used to summarize the population distribution, high-frequency drug use, efficacy ratio, and core prescriptions of each age group. Results The age of onset was highest in patients aged 30-49 years. Core prescriptions for different age groups were obtained. The core prescriptions of 20-<30 years old were stir-frying with vinegar Corydalis Rhizoma, stir-frying with vinegar Cyperi Rhizoma, Typhae Pollen, Angelicae Sinensis Radix, Chuanxiong Rhizoma, Paeoniae Radix Rubra, Paeoniae Radix Alba, stir-frying with salt Foeniculi Fructus; the core prescriptions of 30-<40 years old were stir-frying with vinegar Cyperi Rhizoma, stir-frying with vinegar Corydalis Rhizoma, Typhae Pollen, Angelicae Sinensis Radix, Chuanxiong Rhizoma, Paeoniae Radix Alba, Leonuri Herba, Forsythiae Fructus, stir-frying with salt Foeniculi Fructus; the core prescriptions of 40-<50 years old were stir-frying with vinegar Cyperi Rhizoma, stir-frying with vinegar Corydalis Rhizoma, Leonuri Herba, Chuanxiong Rhizoma, Paeoniae Radix Alba, Typhae Pollen, Ostreae Concha, Angelicae Sinensis Radix, Codonopsis Radix; the core prescriptions of 50-55 years old were Ostreae Concha, Typhae Pollen, Codonopsis Radix, Rehmanniae Radix, stir-frying Astragali Radix, Phellodendri Chinensis Cortex, Ecliptae Herba, stir-frying with wine Ligustri Lucidi Fructus, Anemarrhenae Rhizoma, stir-frying with vinegar Cyperi Rhizoma. Conclusion The onset age of focal AM patients tends to be younger. The treatment of focal AM should be based on activating blood circulation to dissipate blood stasis, regulating qi and relieving pain, and according to the pathogenesis differences of different age groups, with the treatment of warming interior and dispersing cold, clearing heat and detoxifying, replenishing qi and nourishing blood, tonifying kidney and nourishing yin, removing blood stasis and stopping bleeding, etc., to expand the conservative and phased treatment of the disease.

Objective To discuss visual analysis on research progress of acupuncture and moxibustion in the treatment of acute lumbar sprain based on VOSviewer software. Methods The three databases of China National Knowledge Infrastructure, Wanfang Date and VIP were searched, and the relevant literatures on acupuncture and moxibustion in the treatment of acute lumbar sprain was collected from January 1990 to December 2021. The VOSviewer software was used to visually analyze for the content of articles, journals, main researchers, main acupoints, and operation methods. Results A total of 823 literatures were included, and the annual number of literatures reached 51 in 1995, after which the overall trend of decline was slow. The most frequently published journal was Journal of Clinical Acupuncture and Moxibustion, with 65 articles in total, and the core author Wu Yaochi had published seven literatures in total. The highest frequency of occurrence of “hòuxī” in the main acupoints was 100 times. “Massage” was the most frequent way of performing arts, with 51 times. Conclusion The use of new intervention methods to stimulate acupoints or multidisciplinary research on acute lumbar sprain may be the future development trend.

Postsurgical gastroparesis syndrome of digestive tract tumors is one of the common complications after digestive tract tumor surgery, which seriously affects the postoperative quality of life and prognosis of patients. Traditional Chinese medicine has obvious advantages in treating this disease. Based on the theory of “middle jiao as pivot”, Professor Zuo Minghuan believed that this disease in essence is “deficiency of yang qi in middle jiao” and with the appearance of the reality of qi stagnation, blood stasis, and dampness, and that the pathogenesis lies in the above reasons leading to the middle jiao does not function smoothly. Combined with the characteristics of the disease and the theory of external treatment of traditional Chinese medicine, the method of application external treatment was used in the treatment, the selection of warming the middle to disperse cold, activating qi to remove blood stasis and dispelling dampness of the medicine was applied to zhōngwǎn, shénquē point, playing warming and restoring yang qi in middle jiao, dispelling stagnation effect to regulate the center, the balance of the rise and fall, and pay attention to the method of warming the middle, avoid giving the bitter and cold herds such as Natrii Sulfas and Rhei Radix Et Rhizoma to pass down, and achieved good clinical effect. This article introduces Professor Zuo Minghuan’s experience in treating postsurgical gastroparesis syndrome of digestive tract tumors by applying traditional Chinese medicine acupoint application based on the idea of “middle jiao as pivot”, in order to provide more diversified treatment ideas and methods for the clinic.

146 Research on concerted application rule of Chinese patent drug containing Astragali Radix based on data mining

Objective To explore the concerted application characteristics and formulation rules of Astragali Radix in Chinese patent drugs in China at present. Methods Chinese Pharmacopoeia and Traditional Chinese Medicine Prescriptions were used as data sources. Indicative diseases, applicable syndromes, commonly used drug pairs, and concerted application rules of Chinese patent drug containing Astragali Radix were explored by using frequency analysis, association rule analysis, and network atlas analysis. Results Five hundred and seventy kinds of Astragali Radix in Chinese patent drugs were collected and sorted out, involving 624 kinds of traditional Chinese medicines. The drug nature and flavour were mainly warm and sweet, and the channel tropisms were mainly liver, kidney and lung. There were 155 main diseases, mainly including fatigue, dizziness, etc. There were 149 corresponding syndromes, with the highest frequency of syndrome of deficiency of both qi and blood occurring. It was found that common concerted application drugs include Astragali Radix and Angelicae Sinensis Radix through correlation analysis. Sixteen core drug combinations and 15 association rules were obtained. The compatibility network analysis of Chinese patent medicine containing Astragali Radix showed that the core compatibility of Chinese patent medicine containing Astragali Radix showed when the support degree was ≥35%. Analysis on the compatibility of Chinese prescription drugs containing Astragali Radix showed that the core drugs for treating consumptive disease, vertigo, syndrome of deficiency of both qi and blood, and syndrome of deficiency of kidney yang all contained Angelicae Sinensis Radix, Astragali Radix and Rehmanniae Radix Praeparata. Conclusion Astragali Radix in Chinese patent drugs is mainly composed of Astragali Radix with drugs of supplementing qi and blood, tonifying deficiency and nourishing yin, which has prominent therapeutic effect on diseases of syndrome of deficiency of both qi and blood, syndrome of deficiency of kidney yang and syndrome of deficiency of both qi and yin.

151 Study on medication rules of Professor Du Yumao in the treatment of chronic renal failure based on data mining

Objective To explore medication rules of Professor Du Yumao in the treatment of chronic renal failure based on data mining technology. Methods The medical records of chronic kidney failure patients treated by Professor Du from January 1998 to December 2011 were collected and organized based on the medical records of Xianyang Yumao Hospital. The traditional Chinese medicine inheritance assistance platform (V2.5) was applied to statistical analysis of medication frequency, sexual taste and meridian distribution. Association rules and clustering algorithms were used to derive commonly used drug combinations and potential new prescriptions. Results A total of 640 prescriptions that met the research criteria were included, involving 214 traditional Chinese medicines. Among them, 59 flavors with medication frequency more than 40 times. It was found that Professor Du in the treatment of chronic renal failure mainly consists of tonifying deficiency drugs, diuretic and dampness absorbing drugs, and blood activating and stasis resolving drugs. Among them, Atractylodis Macrocephalae Rhizoma was the preferred method for tonifying qi, Epimedii Folium was commonly used for tonifying yang, and Asparagi Radix was commonly used for nourishing yin. The medicinal properties of the medicine were mainly cold and warm, and the medicinal flavors were mainly sweet, bitter, and spicy, mainly classified into the liver and spleen meridians. Through association rule analysis, eight core drugs were obtained, including Atractylodis Macrocephalae Rhizoma, Astr- agali Radix, Poria, Salviae Miltiorrhizae Radix Et Rhizoma, Bupleuri Radix, Scutellariae Radix, Chuanxiong Rhizoma, and Polygoni Cuspidati Rhizoma Et Radix. Fourteen association rules for two drugs were obtained, and 25 association rules for three drugs were obtained. Four potential new parties were identified through cluster analysis. Conclusion This study analyzes the medication rules of Professor Du in the treatment of chronic kidney failure and identifies potential new prescriptions, which have guiding significance for clinical treatment.

In the long-term clinical practice, under the guidance of the holistic view of traditional Chinese medicine and the thought of syndrome differentiation and treatment, combines with western medicine’s understanding of idiopathic membranous nephropathy, Professor Lü Renhe, a traditional Chinese medicine master, summarizes and puts forward the “six diagnosis and treatment methods” for disease differentiation and treatment. Specifically, it includes disease treatment, disease stage syndrome differentiation, disease syndrome differentiation, syndrome differentiation, symptom differentiation, disease differentiation and syndrome differentiation combined. The “six diagnosis and treatment methods” embodies Professor Lü Renhe’s clinical thinking of traditional Chinese medicine, which attaches importance to disease differentiation, stage differentiation of syndrome, combination of disease, and syndrome, and equal emphasis on disease-syndrome-symptom. The application of “six diagnosis and treatment methods” in the differentiation of idiopathic membranous nephropathy can optimize the clinical thinking and improve the clinical effect.

159 Health risk assessment and prediction analysis of elderly population caused by air pollution in Chengdu-Chongqing area in 2020

Objective To quantify the health risks of the elderly population caused by air pollution in Chengdu-Chongqing area and to predict the changes of health risks in the future, so as to provide a theoretical basis for pollution control and health management of the elderly population. Methods The Chengdu-Chongqing area with typical aging was selected as the research area, and the observation data of PM2.5, PM10, NO2, SO2, CO, and O3 were collected from January 1 to December 31, 2020. Health risk assessment and grey prediction model were used to estimate the health risk of the elderly population. Results The atmospheric particulate matter and NO2 showed a “U-shaped” trend of change. The annual concentration of SO2 and CO showed little difference, while O3 pollution showed a trend of “high in summer and low in winter”. The health risk value caused by O3 and CO exceeds the maximum acceptable level of 1×10-6, and the health risk value caused by other pollutants was small. The health risk caused by pollutants in the young elder was greater than that in the very old people. The health risk values of the elderly population were predicted to decrease in the future, and the order from high to low was SO2(86.40%)>PM10(51.58%)>NO2(49.62%)>PM2.5(42.43%)>CO(15.48%)>O3(8.29%). Conclusion The health risk caused by O3 is the most serious in the elderly population and lasts for a long time. In the future, the government should focus on and control O3 pollution, give full play to the function of community health management, and improve the self-health management ability of the elderly.

163 Study on the perioperative nursing of traumatic arthritis of ankle joint after ankle fusion

Objective To explore the perioperative nursing of traumatic arthritis of ankle after ankle fusion. Methods A total of 108 patients with traumatic arthritis of ankle who underwent ankle fusion surgery in the Second Affiliated Hospital of Air Force Military Medical University of the Chinese People’s Liberation Army from May 2016 to April 2021 were selected as examples, and they were divided into control group and observation group by envelope method, with 54 cases in each group. The two groups were given routine nursing, and the observation group was given perioperative nursing on this basis. Ankle dorsiflexion and plantar flexion motion, complications and ankle function were compared between the two groups one day and two weeks after surgery. Results Two weeks after surgery, the ankle dorsalis extension and plantar flexor motion of the two groups were higher than those of the first day after surgery, and those of the observation group were higher than those of the control group, and the differences were statistically significant (P<0.05). The incidence of complications in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). Two weeks after surgery, the ankle joint function score of the two groups were higher than those of the first day after surgery, while that of the observation group was higher than that of the control group, and the differences were statistically significant (P<0.05). Conclusion After perioperative nursing for traumatic arthritis of ankle joint, ankle joint fusion can not only improve the dorsalis extension and plantar flexor motion of ankle joint, but also reduce the risk of postoperative complications and promote the recovery of ankle joint function.

167 Application of patient-centered nursing concept combined with emergency nursing pathway in patients with acute myocardial infarction

Objective To explore the application of patient-centered nursing concept combined with emergency nursing pathway in patients with acute myocardial infarction. Methods Eighty-four patients with acute myocardial infarction admitted to the Department of Emergency, the Second People’s Hospital of Lianyungang, Jiangsu Province from October 2018 to October 2020 were selected. They were divided into reference group and experimental group by random number table method, with 42 cases in each group. The reference group received emergency nursing pathway intervention, and the experimental group received patient-centered nursing concept combined with emergency nursing pathway. The treatment, cardiac function and quality of life of the two groups were compared before and after intervention. Results After intervention, the treatment duration of experimental group was shorter than that of the reference group, and the differences were statistically significant (P<0.05). After intervention, the heart rate, systolic blood pressure, and left ventricular end diastolic diameter of the two groups were lower than those before intervention, and the left ventricular ejection fraction was higher than before intervention, and the heart rate, systolic blood pressure, and left ventricular end diastolic diameter of the experimental group were lower than the reference group, and the left ventricular ejection fraction was higher than the reference group, the differences were statistically significant (P<0.05); the scores of angina quality of life questionnaire in the two groups were lower than those before intervention, and the experimental group was lower than the reference group, and the differences were statistically significant (P<0.05). Conclusion Patient-oriented nursing concept combined with emergency nursing pathway plays an important role in improving patients with acute myocardial infarction. The treatment effect of patients is significantly improved, and the level of new function and quality of life are significantly improved.

Objective To explore the effect of drinking water standard nursing strategy (“JBI strategy” for short) on prevention and control of contrast induced nephropathy (CIN) based on the clinical evidence application system of Joanna Briggs Institute (JBI). Methods Convenience sampling method was used to select 98 elderly patients with lower extremity artery occlusion interventional surgery admitted to the Haian People’s Hospital, Jiangsu Province from January 2018 to January 2021. The patients were grouped according to whether JBI strategy was applied or not, and those who did not apply were included in the conventional group and those who applied were included in the experimental group, 49 cases in both groups. The incidence of CIN, renal function, postoperative dysuria, urinary retention, and stomach discomfort were compared between the two groups. Results The incidence of CIN in experimental group was lower than that in conventional group, and the difference was statistically significant (P<0.05). After surgery, serum creatinine, urea nitrogen and β2 microglobulin levels in two groups were lower than those before surgery, while those of experimental group were lower than those of control group, and the differences were statistically significant (P<0.05). The complication rate of experimental group was lower than that of conventional group, and the difference was statistically significant (P<0.05). Conclusion The drinking water standard nursing strategy based on JBI can effectively reduce the risk of CIN after intervention, and reduce the risk of postoperative stomach discomfort and complications, which has nursing reference value.

Objective To study the effect of graded nutritional intervention based on health quotient assessment on nutritional parameters and phosphorus and calcium compliance rate of patients with end stage renal disease (ESRD) undergoing maintenance hemodialysis. Methods Eighty ESRD patients with maintenance hemodialysis admitted to Bozhou People’s Hospital, Anhui Province from January 2019 to March 2022 were selected as the study subjects. According to random number table method, the subjects were divided into routine group and health quotient nursing group, with 40 cases in each group. The routine group took the routine maintenance hemodialysis nursing mode, and the health quotient nursing group took the graded nutrition management intervention based on the routine group. Nutritional parameters, serum phosphorus, and serum calcium levels were compared between the two groups before and after intervention, and the compliance rate of serum phosphorus and serum calcium after intervention was observed. Results After the intervention, the nutritional risk score of the two groups was lower than that before the intervention, the total protein, hemoglobin, and albumin levels were higher than those before the intervention, and the nutritional risk score of the health quotient nursing group was lower than that of the routine group, and the total protein, hemoglobin, and albumin levels were higher than those of the routine group, and the differences were statistically significant (P<0.05). After intervention, the blood phosphorus and calcium levels in the two groups were lower than those before intervention, and those in the health quotient nursing group were lower than those in the routine group, and the differences were statistically significant (P<0.05). The compliance rate of serum calcium and phosphorus in the health quotient nursing group was higher than that in the routine group, and the differences were statistically significant (P<0.05). Conclusion The graded nutritional intervention based on health quotient assessment in ESRD maintenance hemodialysis patients can effectively improve the nutritional status and blood calcium metabolism of patients.

179 Effect of risk screening combined with precise management model on rehabilitation effectiveness, clinical satisfaction, and quality of life of patients with acute bronchitis in fever outpatient clinic

Objective To study the effect of risk screening combined with precise management model on rehabilitation effectiveness, clinical satisfaction, and quality of life of patients with acute bronchitis in fever outpatient clinic. Methods A total of 150 patients with acute bronchitis in the Fever Clinic, Fuyang People’s Hospital of Anhui Province from March 2021 to March 2022 were selected. According to random number table method, the patients were divided into control group, precision management group and combination group, with 50 cases in each group. The control group was treated with routine nursing intervention, the precision management group was treated with precision management mode intervention on the basis of routine group, and the combination group was treated with risk screening intervention on the basis of precision management group. The rehabilitation effect, nursing quality, life quality and nursing satisfaction of the three groups were observed. Results The time of fever subsiding, cough disappearing, and lung wet rales disappearing in combination group was shorter than that in precision management group, and that in precision management group was shorter than that in control group (P<0.05). The nursing quality scores of the combination group were higher than those of the precision management group, and the precision management group was higher than the control group (P<0.05). After intervention, all scores of generic quality of life inventory in three groups were higher than those before intervention (P<0.05). The generic quality of life inventory scores of the combination group were higher than those of the precision management group, and the precision management group was higher than the control group (P<0.05). The nursing satisfaction of combination group was better than that of precision management group, and precision management group was better than that of the control group (P<0.05). Conclusion Risk screening combined with precision management mode has significant intervention effect on patients with acute bronchitis in fever outpatient department, shortening the rehabilitation process of patients, improving the quality of care, and improving patients’ clinical satisfaction and quality of life.

183 Pharmaceutical care of a patient after liver transplantation treated with Tacrolimus

To explore the positive effect of clinical pharmacists on improving rational drug use in patients after liver transplantation. Clinical pharmacists participated in the drug treatment process of a patient with multiple attempts to remove ventilator and tracheal intubation after liver transplantation, combined with the interaction between immunosuppressants and anti-infective drugs, assist physicians to monitor the blood concentration and interaction of tacrolimus, optimize the drug administration plan, and ensure the safety of individualized drug use for patients. After 37 days of treatment, the patient’s condition was effectively controlled. The ventilator was successfully removed and the tracheal cannula was removed. The patient was transferred to a general ward. Collaboration between clinical pharmacists and doctors can significantly improve drug safety and effectiveness and promote rational drug use through pharmaceutical care of patients.

With the deepening and expansion of brain science research, the brain tissue bank as the infrastructure of brain science research has been attached great importance by countries all over the world. This paper summarizes the construction and management of human brain tissue banks in the United States, the United Kingdom, the Netherlands, and Japan, as well as the experience and characteristics in the aspects of ethical standards, operational norms, diversified funding sources, high-quality brain bank management, data sharing platform construction, and analyzes the current situation of the construction of human brain tissue banks in China. At the same time, combining with the needs of Chinese brain science research, this paper draws on foreign experience, and puts forward some policy suggestions on the construction and management of Chinese brain bank from the aspects of strengthening policies and regulations, increasing financial investment, strengthening the construction of data sharing platform, promoting international cooperation and governance, strengthening ethics and humanistic care, etc.

The public health emergency management system not only involves different subjects such as the government, patients, medical institutions, and center for disease control and prevention, but also is affected by policies and laws, it is a complex system engineering, problems in one aspect will affect the operation of the entire system. From the perspective of actor network theory, this paper analyzes the operation process of China’s public health emergency management system through translation steps, sorts out the general relationship between different actors, and summarizes the current public health emergency management system in my country. The obstacles and benefits encountered by heterogeneous actors, and suggestions for improving the operation mechanism of my country’s current public health emergency management system are discussed.
