DOI: 10.20047/j.issn1673-7210.2023.13.01
Objective To explore the approach of acupotomy release rabbit lumbar ligamentum flavum under ultrasound guidance, according to the anatomical structure of the rabbit lumbar spine and the shape of the ligamentum flavum. Methods The height of spinous process, mastoid process, length, and width of lamina space of L5, L6, and L7 vertebrae in six male New Zealand rabbits aged three months were measured; the six 3-month-old male New Zealand rabbits were measured with the ultrasound probe vertical. The lateral axis of the posterior midline was placed, and the angle between the spinous process and the mastoid process was measured under ultrasound. The longitudinal axis of the ultrasonic probe was placed beside the spinous process to release the ligamentum flavum on both sides of L4/5, L5/6, and L6/7 segments in a plane way the acupotomy insertion depth, insertion angle, the vertical distance from the insertion point to the posterior midline and the vertical distance from the horizontal line of the sacral angle were measured, after the measurement, the data was analyzed and the anatomical structure of acupotomy path was observed. Results There was no significant difference in mastoid hight and spinous process height among L5, L6, and L7 vertebrae (P>0.05). The length and width of the L5-L7 intervertebral space gradually increased, and the middle part of the yellow ligament was thicker, followed by the medial part, and the lateral part was thinner. There was no significant difference in the angle between spinous process and mastoid process on both sides of L5, L6 and L7 vertebrae measured by ultrasonic transverse axis (P>0.05). After the longitudinal approach of the needle knife arrived at the yellow ligament under ultrasound guidance, there was no statistical significance in the vertical distance between the insertion points of the left and right sides of the L4/5, L5/6 and L6/7 segments measured after needle retention, the insertion depth of the needle knife, the angle between the needle knife and the horizontal line, namely the insertion angle of the needle knife (P>0.05). The vertical distance of the horizontal line of the sacral angle at the left and right needle insertion points of L6/7 was shorter than L4/5 and L5/6, and the vertical distance of the horizontal line of the sacral angle at the left and right needle insertion points of L5/6 was shorter than L4/5, the differences were statistically significant (P<0.05). No obvious blood vessels and nerves were observed in the puncture path, and the dura was intact. Conclusion It is a safe approach to release the rabbit ligamentum flavum with acupotomy when the ultrasound probe is placed parallel to the longitudinal axis. The needle insertion angle should be adjusted according to the anatomical differences of L4/5, L5/6, and L6/7 segments.

41 Research progress on tongue image of traumatic blood stasis syndrome
KONG Jiaxing DENG Rongrong XU Xidong ZHAO Xu KANG Ran
DOI: 10.20047/j.issn1673-7210.2023.13.09
The etiology and pathogenesis of traumatic blood stasis syndrome is clear, and there is no cold and heat and yin and yang bias in the early onset, which is more conducive to the objective study of tongue symptoms of blood stasis syndrome. The analysis of the characteristics of the tongue image mainly focuses on three aspects: ecchymosis (petechia), (blue) purple tongue, and sublingual collaterals, with the highest diagnostic weight, which can indicate the severity of blood stasis syndrome and help to judge the occurrence and development of the disease. At the same time, some results have been achieved in the objective analysis of tongue characteristics of traumatic blood stasis syndrome, but the image definition of ecchymosis (petechia) on the tongue, the chromaticity value range of (blue) purple tongue, and the sublingual collaterals in terms of segmentation and extraction, there are problems such as inconsistent standards, unsatisfactory accuracy, and stability. In addition, the tongue characteristics of traumatic blood stasis syndrome are closely related to blood indicators and immune indicators, which can reflect blood stasis and hypercoagulability, and have broad application prospects in clinical diagnosis and treatment of diseases.

58 Study of correlation between intestinal flora and preeclampsia
ZHANG Jiwei1 JIA Sihui1 QIAO Jing1 WANG Cuixiu2
DOI: 10.20047/j.issn1673-7210.2023.13.13
Preeclampsia is a unique complication during pregnancy, which seriously threatens the health of mothers and fetuses. Up to now, the etiology and pathogenesis have not been fully elucidated, but a large number of studies have shown that the pathogenesis may be related to inflammatory immune overactivation, abnormal glucose and lipid metabolism, oxidative stress, and other abnormalities. With the in-depth study of intestinal flora, studies have found that intestinal flora can affect the over-activation of inflammatory immunity by regulating inflammatory factors, cytokines, and the proliferation and differentiation of T cells. When intestinal flora is disturbed, short-chain fatty acids produced by metabolism are reduced, which affects glucose and lipid metabolism and jointly participate in the occurrence and development of preeclampsia. In addition, placental microecology can be a new entry point to explore the pathogenesis of preeclampsia. This provides new ideas for revealing the pathogenesis of preeclampsia and exploring the prevention and treatment of preeclampsia. This article reviews the current research on the correlation between intestinal flora and preeclampsia.

Foreign countries have formed a multi-dimensional skill training system with comprehensive curriculum model, standardized training model, diversified training content, visualization of training results, and standardized assessment model, while domestic medical colleges put most of their energy into the operation training of skill assessment while ignoring the systematic training of laparoscopic skill. As a result, there is a big gap between the training mode and effectiveness of laparoscopy skill at home and abroad. In this paper, the reform of laparoscopic operation skill training mode is carried out through five aspects, including the reform of curriculum mode, the reform of training mode, the reform of enriching training content, the reform of visual training process, and the reform of comprehensive examination mode, so as to construct a “multidimensional and progressive” laparoscopic skill training system suitable for Chinese clinical medical students. In order to cultivate all-round medical talents with excellent medical literacy and solid operational basic skills.

131 Experience of Professor Huang Shijing’s differential treatment of white matter ischemic lesions based on the theory of brain and kidney
CHEN Yuxia YU Bin PAN Juhua HUANG Shijing
DOI: 10.20047/j.issn1673-7210.2023.13.30
White matter ischemic lesions is a common pathological change in the brain of the elderly, which is closely related to hypertension, cognitive decline, and ischemic stroke. At present, no effective intervention has been found. Professor Huang Shijing has observed and practiced for many years. It is concluded that the pathological basis of the occurrence and development of ischemic leukoencephalopathy is “kidney deficiency, phlegm blocking blood stasis”, and “brain and kidney failure” is an important part of its pathogenesis. Centering on this pathogenesis, it is proposed that special attention should be paid to the role of “communicating brain and kidney” in treatment on the basis of “tonifying kidney and filling marrow, resolving phlegm and removing blood stasis”. A lot of clinical results have been achieved, which provides a new idea for the prevention and treatment of many kinds of encephalopathies. This paper systematically expounds Professor Huang Shijing’s experience in differential diagnosis and treatment of ischemic leukoencephalopathy based on the theory of brain and kidney.

DOI: 10.20047/j.issn1673-7210.2023.13.33
Abnormal taste refers to the abnormal taste and odor in the human body. Western medicine is easy to produce drug resistance, and the drug is easy to relapse, and there are certain limitations. Professor Liu Deshan believes that the pathogenesis of abnormal taste is always attributed to the dysfunction of the spleen and stomach or other zang-fu organs, which is a symptom of the disease of the zang-fu organs reflected in the mouth through the meridians and collaterals. Sweet taste is often treated from the dampness and heat of the spleen and stomach, sour taste is often treated from the stagnation of the spleen and stomach, bitter taste is often treated from the stagnation of the bile organs, and spicy taste is often treated from the heat of the lung and stomach. In addition, attention should be paid to grasp the differentiation of cold and heat, deficiency and excess when the syndrome is present. With the addition and subtraction of the syndrome, the disease is treating both manifestation and root cause, which have good effect, then broaden the idea for the treatment of abnormal taste. This paper summarizes clinical thinking of Professor Liu Deshan in the treatment of sweet taste, sour taste, bitter taste, and spicy taste, in order to provide experience for treatment of abnormal taste by traditional Chinese medicine. Last but not least, a typical medical case is attached for peer’s reference.

DOI: 10.20047/j.issn1673-7210.2023.13.36
Objective To evaluate the risk of immune-associated encephalitis (AIE) caused by immune checkpoint inhibitors (ICI) based on the US Food and Drug Adverse Reaction Reporting System. Methods Adverse event reports of ICI (including Navulizumab, Parbolizumab, Attilizumab, Abvizumab, Duvarizumab, Cemiplimab, Ipimimumab,  and Tremelimumab) were collected from FAERS database from 1Q2011 to 4Q2021. The risk, occurrence time, patient outcome,  and ICI reintroduction of AIE adverse events were evaluated by ROR. Signal was considered to exist when the number of reported adverse events related to the target drug was ≥ 3 and the lower limit of 95% ROR signal was ≥ 1. Results A total of 865 adverse events of AIE were reported, involving 234 patients receiving ICI therapy. The signal emerged in Nivolumab, Pembrolizumab, Atezolizumab, Durvalumab, Nivolumab with Ipilimumab, and durvalumab with Tremelimumab, with Durvalumab having the strongest signal (ROR 95% CI: 40.97 [25.70-65.31]). Nivolumab plus ipilimumab had a higher risk of AIE compared to Nivolumab monotherapy (ROR 95% CI: 33.16 [25.49-43.15]), with a shorter period of onset of time(median 60 days). More than 70% of patients with ICI-associated AIE required hospitalization or prolonged hospital stay, with a fatality rate of 18.80% (44 cases). No reports of ICI resumption were collected. Conclusion The risk of AIE induced by ICI is different, and ICI-associated AIE tended to occur within three months after ICI treatment, which should attract clinical attention.
