HU Jing WEI Wan GAO Hui XU Gang YANG Huayuan
Objective To investigate the regulation of prostaglandin E receptor 2 (EP2) / Nod-like receptor protein 3 (NLRP3) and its anti-inflammatory mechanism in primary dysmenorrhea rats by sparrow-pecking moxibustion. Methods A total of 24 female SD rats were divided into blank group, model group, sprrow-pecking moxibustion group, and agonist group, with six rats in each group. Except blank group, the primary dysmenorrhea model was established for the other three groups, blank group received the same dose of normal saline daily. Blank group and model group were not treated; in sparrow-pecking moxibustion group, sānyīnjiāo and gu?觀nyuán acupoints were selected for treatment after daily modeling; agonist group was intraperitoneally injected with Treprostinil daily after modeling, followed by sparrow-pecking moxibustion treatment, which was the same as the sparrow-pecking moxibustion group. The latent time and number of torsion were observed in each group; HE staining was used to observe the pathological conditions of uterus; the expressions of EP2, NLRP3, and caspase-1 proteins were detected by immunohistochemistry. Results Compared with blank group, the latent time was shortened and the number of torsion was increased in model group (P < 0.01); compared with model group, the latent time was prolonged and the number of torsion was decreased in sparrow-pecking moxibustion group (P < 0.01); compared with sparrow-pecking moxibustion group, the latent time was shortened and the number of torsion was increased in agonist group (P < 0.01). In blank group, the uterine tissue morphology was regular, there were a few lymphocytes in the interstitium of muscle cells, and no neutrophil infiltration; in model group and agonist group, small uterine vessels showed dilation and hyperemia, and inflammatory cells such as lymphocytes and eosinophils were infiltrated outwards as focal groups; in sparrow-pecking moxibustion group, the degree of interstitial hyperemia and edema and infiltration of inflammatory cells were less than that in model group, a few inflammatory cells infiltrated between muscle walls and only a few small blood vessels hyperemia were observed. Compared with blank group, the protein expressions of EP2, NLRP3, and caspase-1 were increased in model group (P < 0.01); compared with model group, the protein expressions of EP2, NLRP3, and caspase-1 were decreased in sparrow-pecking group (P < 0.01); compared with sparrow-pecking group, the protein expressions of NLRP3 and caspase-1 were increased in agonist group, while the protein expression of EP2 in sparrow-pecking group and agonist group was not significantly different (P > 0.05). Conclusion Sparrow-pecking moxibustion can improve the infiltration of inflammatory cells in the uterus, and regulate the inflammatory response of dysmenorrhea rats by regulating EP2/NLRP3 signal pathway.

42 Application progress of ultrasound elastography in breast diseases
DONG Meng ZHANG Bingyi XING Boyuan
At present, the incidence and mortality of breast cancer show a significant upward trend. Early detection, diagnosis and treatment are very important for breast cancer patients. Two-dimensional ultrasound has low specificity in diagnosing breast cancer, and ultrasound elastography (UE) can evaluate the hardness characteristics of breast tissue on the basis of two-dimensional ultrasound, thereby providing effective information for accurate diagnosis and treatment of breast cancer. UE techniques are mainly divided into two categories: strain elastography and shear wave elastography. The clinical application of UE is not only reflected in the identification of benign and malignant breast nodules, but also has great value in adjusting the BI-RADS classification, monitoring the efficacy of neoadjuvant chemotherapy, and evaluating the prognosis of breast cancer patients. This paper reviews the basic principles, diagnostic methods and clinical applications of breast diseases of UE.

50 Research progress on the effect of SGLT2 inhibitors on osteoporosis in type 2 diabetes mellitus
LIU Siqi1 GUO Ying2 ZHANG Chunlan2 ZHAO Na2 WANG Juanli3

[Abstract] Osteoporosis is a common complication of diabetes mellitus patients, which seriously affects the quality of life of patients. As a new type of hypoglycemic drug sodium-glucose transporter 2 (SGLT2) inhibitors are widely used in clinical, their effects on bone metabolism have also received widespread attention. Although the drug has good performance in lowering glucose and blood pressure, it may affect bone metabolism and increase the risk of fracture. In order to better guide the clinical use of SGLT2 inhibitors, related factors of osteoporosis caused by SGLT2 inhibitors were described in detail by referring to relevant literatures at home and abroad. The influence of bone metabolism, osteoporosis and even fracture may be related to the imbalance of calcium and phosphorus homeostasis and the increase of parathyroid hormone. However, it is still controversial whether the above consequences are actually related to the use of SGLT2 inhibitors. For diabetic patients with high fracture risk, it should be the consensus of clinicians to fully evaluate the risk of fracture before using these medications.

54 Research progress on rationality of lymph node status involved in cervical cancer staging system
CHANG Hong WU Yumei
Cervical cancer is the most common malignant tumor of the female reproductive system. In the past, clinical staging was used, in 2018, the International Federation of Obstetrics and Gynecology first proposed to set the lymph node metastasis diagnosed by imaging and pathology as stage ⅢC, reflecting that lymph node positivity was independent risk factors for prognosis of patients with cervical cancer. At the same time, the number of lymph node metastases, the size of the local tumor and the degree of parametrial invasion are also important determinants of prognosis. Imaging can diagnose lymph node metastasis by color doppler ultrasound, CT, MRI, PET/CT, etc. After lymphadenectomy whether or not combine abdominal aortic lymphadenectomy, patients with locally advanced disease can also undergo pathological diagnosis of lymph node metastasis after retroperitoneal lymphadenectomy. Concurrent chemoradiotherapy is recommended for the treatment of patients with confirmed lymph node metastasis. This article reviews the studies on the involvement of lymph node status in the staging of cervical cancer.

83 Study on strategies of maintaining national culture security via English teaching in the universities of Chinese medicine
YAN Li1 KONG Xiangguo1 CHENG Wang2 WANG Shanshan1
In the process of internationalization of higher education, the overt communication and implicit penetration of western ideology and its discourse system, to some extent, has impacted Chinese traditional culture and value system, making Chinese culture security face both international and domestic challenges. The study on how college English teaching undertakes the important mission of safeguarding national culture security would contribute to the current construction of “national consciousness” and the in-depth practice of “course ideology and politics” in foreign language education. This paper expounds the status quo and problems of college English teaching and puts forward four strategies to maintain Chinese culture security, taking Chinese medicine universities for example. It points out that deepening the teaching of traditional Chinese medicine culture related courses, highlighting the traditional Chinese medicine characteristic culture content, strengthening the appreciation of traditional Chinese medicine philosophy and culture, and enhancing the cultural demands of English learning from the internal and external variables of curricula, textbooks, teachers, and students respectively should be taken, in order to realize the reconstruction of college English teaching as an ecological system and achieve the “win-win” outcome of Chinese cultural communication and Chinese-western cultural exchange.

110 Discussion on the characteristics of traditional Chinese medicine pathogenesis of early liver cirrhosis of chronic hepatitis B
ZHAI Changming1 LU Fang1 GAO Ya2
Hepatitis B virus (HBV) infection is the most common cause of liver cirrhosis in China. However, due to the long-term and occult progression of liver cirrhosis, the diagnosis and treatment of early liver cirrhosis is still a world problem to be solved. From the perspective of traditional Chinese medicine, HBV should be the evil of dampness and heat, the deficiency of vital qi in the body is an important cause of chronic HBV infection, and phlegm turbidity and blood stasis are the pathological basis of liver fibrosis/cirrhosis, and “fire, dampness, deficiency, phlegm and blood stasis” are the core pathogenesis of early liver cirrhosis. Compared with modern medicine, traditional Chinese medicine has its unique advantages. It can apply decoction based on syndrome differentiation according to the pathogenesis, and intervene in advance through non-drug means such as acupuncture, massage and guidance, so as to jointly play the role of “prevention before disease and treatment before disease”.

114 Li Dake’s experience in treating remitting seronegative symmetrical synovitis with pitting edema based on the theory of phlegm, fluid retention, water and qi disease
SUN Zirui1 LI Dake2 WANG Shiyuan3
As a rare rheumatic disease with no strict and unified diagnostic criteria, remitting seronegative symmetrical synovitis with pitting edema is characterized by sudden appearance of symmetrical edema at the end of the extremities. Currently, most modern physicians use drugs empirically, and think that the pathogenesis is arthralgia caused by damp-heat pathogen, but there are still some patients who have poor efficacy from the perspective of clearing heat and promoting diuresis. At present, Professor Li Dake analyzes the pathogenic characteristics of phlegm, fluid retention, water and qi disease, and tries to describe the pathogenesis of this disease with the theory of phlegm, fluid retention, water and qi disease. She maintaines that the pathogenesis of this disease results from the lung qi failing in dispersing and the fighting of wind with water; dysfunction of spleen in transportation and fluid-dampness stagnancy; deficiency of kidney yang and dysfunction of qi in transformation; dysfunction of sanjiao and imbalance of water qi. It can be treated by ventilating lung qi for diuresis to generate body fluid, invigorating spleen and eliminating dampness, to promote transportation, warming kidney yang to assist qi in transformation, and regulating sanjiao and smoothing waterway, so as to give full play to the role of traditional Chinese medicine in preventing and treating remitting seronegative symmetrical synovitis with pitting edema syndrome, and open up new ideas for clinical diagnosis and treatment and case specific diagnosis and treatment strategies.

QU Jing ZHI Nan LIAN Haihong CAO Shan
The method of clearing lung and invigorating spleen is an innovative method for the treatment of allergic rhinitis with lung heat and spleen dampness syndrome based on inheritance and practice. This paper combs the occurrence of allergic rhinitis with lung heat and spleen dampness syndrome, that is, the insidious pathogen of dampness heat is the internal basis, exuberance of lung heat, spleen deficiency and dampness aggregation, dampness heat cementation, and invading nasal orifices are important pathogenesis. It is pointed out that it is mainly due to the lung, related to the spleen, and the pathogenic factors are heat and dampness cementation. Therefore, the method of clearing lung and invigorating spleen is proposed. The specific herbs are analyzed from two angles, one is the primary and secondary drugs, which are different from monarch, minister, assistant and envoy; secondly, in terms of nature and flavour, it is divided into three categories: cold herbs clearing lung heat, tasteless flavor herbs invigorating spleen for promoting diuresis and spicy warm herbs dispelling wind for opening orifices. After years of practice and clinical research, this prescription has been proved to be safe and effective. This time, the key points of syndrome differentiation of lung heat and spleen dampness syndrome are summarized, which is convenient for clinical popularization. In the diagnosis and treatment of patients, the commonly used adjusted herbs are sorted out under the guidance of theory in different specific situations. Finally, an example of medical records is shown.
