Effects of serum exosomes in intermittent hypoxia mice on epithelial-mesenchymal transformation, invasion and metastasis of Lewis lung cancer cells

Objective To investigate the effect of serum exosomes in intermittent hypoxia (IH) mice on epithelial-mesenchymal transition, invasion and metastasis of Lewis lung cancer cells (LLC). Methods Eighty C57BL/6J male mice, 7-8 weeks old, weighing 22-25 g were selected. Two mice with similar body weight were randomly divided into indoor air group (RA) and IH group. The difference in average body weight of mice between the two groups was ≤2 g, with 40 mice in each group. In the IH group, IH stimulation was performed for one minute per cycle, and the oxygen partial pressure was changed from (6±1)%/30 s to (21±1)%/30 s, 12 h/d, for four consecutive weeks. Mice in RA group were housed in ordinary cages. Extraction of exosomes from mouse serum; cell viability was detected by CCK-8 method; Transwell assay and cell scratch assay were used to detect cell invasion and migration ability; Western blot was used to detect the protein expression levels of E-cadherin, N-cadherin, and matrix metalloproteinase 9 (MMP-9). Results After incubating LLC cells with mouse serum exosomes, the number of viable cells in the IH group was higher than that in the RA group, and the difference was highly statistically significant (P < 0.01); the ability of cell invasion and migration in the IH group was higher than that in the RA group, and the difference was highly statistically significant (P < 0.01); the expression level of E-cadherin in the IH group was lower than that in the RA group, and the difference was statistically significant (P < 0.05); the expression level of N-cadherin in the IH group was higher than that in the RA group, and the difference was statistically significant (P < 0.05); therei was no significant difference in the expression levels of MMP-9 between the two groups (P > 0.05). Conclusion Serum exosomes from IH mice may induce EMT and invasion and metastasis of LLC cells.

Discussion on medication rules in the treatment of distention and fullness based on ancient medical records

Objective To summarize the medication rules of ancient physicians in the treatment of distention and fullness. Methods The ancient distention and fullness medical records in Yian Leiju were collected and sorted out, the data of prescriptions that met the inclusion criteria were standardized, the database of prescriptions were established, and SPSS Statistics 26.0 was used to conduct descriptive statistical analysis, high frequency drug cluster analysis, and factor analysis. Results A total of 163 cases of medical records of distention and fullness, 212 consultations, 212 prescriptions of traditional Chinese medicine, involving 196 flavors of traditional Chinese medicine were obtained. High frequency drugs included Pinelliae Rhizoma, Poria, Citri Reticulatae Pericarpium and so on. Drug properties were mainly warm; drug tastes were mainly pungent, sweet and bitter; channel tropism were mainly spleen, stomach and lung channel. High frequency drug cluster analysis was divided into six categories, and nine common factors were extracted from factor analysis. Conclusion Ancient physicians treat distention and fullness with tonifying deficiency and dispelling excess, in which tonifying deficiency is mainly to tonify spleen qi and warm spleen yang; dispelling excess is mainly to dissipate phlegm and dampness, soothe liver qi, and eliminate food stagnation.

Research on the medication rules of Professor Jiang Hongjiang in the treatment of knee arthralgia disease based on data mining technology

Objective To study Professor Jiang Hongjiang’s medication rules and academic characteristics in the treatment of knee arthralgia disease based on the method of prescription data mining, and pass it on in a timely manner. Methods The medical records of knee arthralgia disease treated by Professor Jiang Hongjiang in Shandong Wendeng Osteopathic Hospital from September 2020 to April 2021 were collected and the prescription data were collected by a designated person. The database of medical records of “Professor Jiang Hongjiang’s experience in knee treatment” was established on the ancient and modern of cloud platform of medical records. Data mining analysis was used to analyze the frequency, nature and flavour, and channel tropism, and efficacy of traditional Chinese medicine in medical records, and correlation analysis, complex network analysis and cluster analysis were used to obtain the compatibility rules, core combination, and core cluster of traditional Chinese medicine. Results A total of 82 medical records were collected, including 68 traditional Chinese medicine. The top three traditional Chinese medicine frequency were Achyranthis Bidentatae Radix, Gardeniae Fructus Praeparatus, and Chuanxiong Rhizoma. The drug properties were mainly flat and warm; the drug tastes were mainly bitter, sweet and bitter; channel tropism were mainly liver, lung, kidney, heart, and spleen channel; the effect was mainly to tonify liver and kidney, strengthen muscles and bones, dispel wind and relieve pain. From the compatibility and association rules of traditional Chinese medicine, it could be concluded that the traditional Chinese medicine combinations mainly include tonifying liver and kidney, promoting blood circulation for removing blood stasis, removing dampness and relieving pain. Through the complex network analysis, the core recipe combination was Achyranthis Bidentatae Radix, Gardeniae Fructus Praeparatus, Angelicae Sinensis Radix, salt Eucommiae Cortex, Astragali Radix, Dipsaci Radix piece, Chuanxiong Rhizoma, Luffae Fructus Retinervus, stir-fried Myrrha, and Forsythiae Fructus. Four core clustering groups were obtained by clustering analysis. Conclusion Professor Jiang Hongjiang’s main treatment methods for knee arthralgia disease are tonifying liver and kidney, strengthening muscles and bones, dispelling wind and relieving pain, promoting blood circulation and removing stasis, which is of great benefit to clinical medication and experience inheritance.

Effect of First Prescription for Hemorrhagic Stroke on the brain tissue of rats with intracerebral hemorrhage

Objective To study the effect of First Prescription for Hemorrhagic Stroke on the brain tissue of rats with intracerebral hemorrhage (ICH). Methods A total of 84 male SD rats with the body weight of (220±20) g were randomly selected and divided into blank group and sham operation group (injected with normal saline) by random number table method, with 12 rats in each group. The remaining 60 rats were treated with ICH model induced by collagase Ⅶ injection, and were divided into model group, Naoxueshu group (2.08 ml/kg), and First Prescription for Hemorrhagic Stroke low-dose, medium-dose, and high-dose groups (3.2, 6.4, 12.8 g/kg) by random number table method. Blank group, sham operation group, and model group were intragastric with equal amount of normal saline, and the other groups were intragastric with corresponding dose of drugs, once a day, for consecutive seven days. After administration, hematoxylin-eosin staining was used to observe the pathological changes of seven groups. The changes of cerebral water content in seven groups were determined by wet and dry weight method. The levels of tumor necrosis factor -α (TNF-α) and interleukin-6 (IL-6) in the seven groups were detected by enzyme linked immunosorbent assay, and the levels of nuclear factor-κB (NB-κB) p65 and phosphorylated NF-κB (p-NF-κB) p65 were detected by Western blot. Results The blank group and the sham operation group had complete brain tissue structure, the model group had a large number of red blood cells and inflammatory cells infiltration, disorder, severe edema, First Prescription for Hemorrhagic Stroke low-dose, medium-dose, and high-dose groups all had a small number of red blood cells. There were no significant differences in cerebral water content, TNF-α, IL-6, and p-NF-κB p65 levels between blank group and sham group (P > 0.05). The cerebral water content, TNF-α, IL-6, and p-NF-κB p65 in model group were higher than those in blank group (P < 0.05). The cerebral water content, TNF-α, IL-6, and p-NF-κB p65 in First Prescription for Hemorrhagic Stroke medium-dose and high-dose groups were lower than those in the model group and First Prescription for Hemorrhagic Stroke low-dose group (P < 0.05). There were no significant differences in cerebral water content, TNF-α, IL-6, and p-NF-κB p65 levels between First Prescription for Hemorrhagic Stroke low-dose group and model group (P > 0.05). There was no significant difference in cerebral water content, TNF-α, IL-6, and p-NF-κB p65 between Hemorrhagic Stroke medium-dose and high-dose groups (P > 0.05). There was no significant difference in NF-κB p65 level among all groups (P > 0.05). Conclusion Medium and high dose of First Prescription for Hemorrhagic Stroke has brain protective effect on ICH rats, which may be related to the inhibition of p-NF-κB P65, TNF-α, and IL-6 expression.

Visual analysis of research status and hot spots of specialized nurse training in China based on CiteSpace software

Objective To understand the current situation and trend of specialist nurse training in China, and to identify the hotspots and frontiers of related research, so as to provide ideas and directions for the development of specialist nurse training. Methods The literatures on specialist nurse training published from January 1998 to December 2020 in CNKI were searched. CiteSpace was used to make a visual analysis. Researching hotspots in the field of specialty nurses were presented. Corresponding knowledge maps were drawn and analyzed. Results A total of 2210 effective documents were retrieved, and the annual publication volume showed a upward trend, with relatively scattered research institutions and less institutional cooperation. The research focuses on core competency training, Delphi method, nursing management, ICU, operating room, diabetes, oncology training, and other areas. The future development direction of specialty nurse training research is evaluation index, continuing nursing, post competence and so on. Conclusion The training of specialized nurses started late in China. Although the research content is increasingly diversified in recent years, the utilization rate and practice scope of specialized nurses need to be improved and expanded, and the research efforts in this field still need to be strengthened.

Relationship of serum miRNA-196a and miRNA-92a expression levels with recurrence of cervical cancer after radical surgery

Objective To investigate the relationship between the expression levels of serum microRNA (miRNA)-196a and miRNA-92a and recurrence of cervical cancer after radical surgery. Methods A total of 113 patients undergoing radical resection of cervical cancer who were admitted to Gansu Provincial Cancer Hospital from January 2015 to June 2016 were selected as the cervical cancer group, qRT-PCR was used to detect serum miRNA-196a and miRNA-92a levels in the both groups. The relationship between serum miRNA-196a and miRNA-92a levels and clinicopathological characteristics of cervical cancer patients was analyzed. Post-operative follow-up for five years, and the cervical cancer patients were divided into high expression and low expression according to the mean value of serum miRNA-196a and miRNA-92a. The recurrence rates of cervical cancer patients in high expression and low expression were compared, multivariate Cox regression was used to analyze the influencing factors of cervical cancer after radical surgery. Receiver operator characteristic (ROC) curve was used to analyze the predictive value of serum miRNA-196a and miRNA-92a levels on recurrence of cervical cancer after radical surgery. Results The expression levels of serum miRNA-196a and miRNA-92a in cervical cancer group were higher than those in control group, and the differences were statistically significant (P < 0.05). There were statistically significant differences in serum miRNA-196a and miRNA-92a levels among cervical cancer patients with different Federation of Gynecology and Obstetrics (FIGO) stages, interstitial invasion depth, and lymph node metastasis (P < 0.05). By the last follow-up, 25 patients (22.12%) of 113 cervical cancer patients had recurrence after surgery and the recurrence rate of miRNA-196a and miRNA-92a of the high expression were higher than those of the low expression, and the differences were statistically significant (P < 0.05). FIGO stage ⅡA, interstitial invasion depth ≥1/2, lymph node metastasis, miRNA-196a ≥5.239, and miRNA-92a ≥1.816 were the independent risk factors for recurrence on cervical cancer after radical surgery (HR > 1, P < 0.05). ROC curve showed that the area under the curve of miRNA-196a combined with miRNA-92a in predicting the recurrence on cervical cancer after radical surgery was greater than that of both alone (Z = 2.319, 2.582, P = 0.020, 0.010). Conclusion Serum miRNA-196a and miRNA-92a levels in cervical cancer are high expression, both of which are associated with FIGO stage, depth of interstitial infiltration, lymph node metastasis, and recurrence, and can be used as predictors of recurrence on cervical cancer after radical surgery.

Expressions of VEGF, Hsa-miR-93, and S100A8 in breast cancer and their relationship with prognosis

Objective To analyze the expressions of vascular endothelial growth factor (VEGF), Hsa-miR-93, and S100 calcium binding protein A8 (S100A8) in breast cancer and their relationship with prognosis. Methods The cancer tissues and corresponding para-carcinoma nomal tissues were collected from 77 patients pathologically confirmed with new breast cancer who were treated in the Chizhou People’s Hospital from January 2017 to June 2018. The expressions of VEGF and S100A8 in tissues were detected by immunohistochemistry; the expression of Hsa-miR-93 was detected by in situ hybridization method; the relationship between VEGF, Hsa-miR-93, S100A8 levels and clinicopathological characteristics of patients were analyzed; according to patients’ survival data of three years after surgery, the effects of VEGF, HSA-miR-93, and S100A8 expression levels on prognosis and survival were analyzed. Results The positive expression rates of VEGF, Hsa-miR-93, and S100A8 in breast cancer tissues were higher than those in para-carcinoma nomal tissues (P < 0.05). The positive expression of VEGF in cancer tissues was related to lymph node metastasis (P < 0.05), while not related to age, histological grading, TNM staging or tumor diameter (P > 0.05); the positive expression of Hsa-miR-93 in cancer tissues was related to TNM staging and lymph node metastasis (P < 0.05), while not related to age, histological grading or tumor diameter (P > 0.05); the positive expression of S100A8 in cancer tissues was related to histological grading, TNM staging, and lymph node metastasis (P < 0.05), while not related to age or tumor diameter (P > 0.05). There was no significant difference in survival rate between patients with positive expression of VEGF, HSA-miR-93, and S100A8 in breast cancer tissues and patients with negative expression (P > 0.05). Conclusion VEGF, miR-93, and S100A8 are highly expressed in breast cancer tissues, but there is no significant difference in positive expression rate and survival rate.

Research progress on anticancer effect and mechanism of isorhamnetin

Tumor is still an important factor that threatens human health and increases the social medical burden. The search for plant-derived antitumor drugs with high efficiency and low toxicity is considered to be an important idea for effective treatment and improving the quality of life of tumor patients. Isorhamnetin is a natural small-molecule flavonoid that widely exists in a variety of plants, and has various biological functions such as anti-inflammatory, anti-viral, anti-myocardial hypoxia-ischemia, and lipid-lowering. Recent studies have found that isorhamnetin can act as an anti-tumor drug to exert anti-cancer effects in a variety of tumor cells, and its mechanism of action mainly includes promoting cancer cell apoptosis, inducing cancer cell autophagy, inhibiting its invasion and metastasis, enhance chemotherapy sensitivity, and other processes. This article reviews the anticancer effect and mechanism of isorhamnetin, aiming to provide new ideas and strategies for future research on the anticancer mechanism of isorhamnetin and tumor therapy.

Research advance of microbial communities of lung and intestinal in asthma

Asthma is a common chronic respiratory disease with an increasing incidence year by year. Its clinical characteristics, immunology, and molecular etiology often vary from person to person. In recent years, with the rise of microbial metabolomics analysis, it has been found that microbial communities of lung and intestinal play an important role in the development, regulation, and maintenance of healthy immune response, thus giving birth to the concept of “entero-pulmonary axis”. Microbial communities of lung and intestinal affects the development, clinical features, and treatment response of asthma. The effects and mechanisms of different microbial communities on bronchial asthma are still unclear. In this paper, we review the relationship between microbial communities of lung and intestinal and their effects on asthma, providing new ideas for the diagnosis and personalized treatment of asthma.

Research progress of traditional Chinese medicine treatment of chronic kidney disease based on real world

Real-world study (RWS) has various types and wide applications, which can reflect the actual clinical scene. In recent years, China has frequently issued relevant policies to vigorously support the development of RWS. Traditional Chinese medicine is widely recognized in the treatment of chronic kidney disease, and has accumulated a lot of experience in the real world. Based on the definition and development of RWS, this article introduces the clinical application of traditional Chinese medicine in the treatment of chronic kidney disease, evaluates the true efficacy, safety, prognosis, and target prediction of traditional Chinese medicine, and explores better practices, in order to provide reference for more related research.

Analysis on the awareness rate and influencing factors of colorectal cancer prevention and control knowledge among a rural resident in Beijing under the “Chinese and Western medicine prevention and control model”

Objective To understand the awareness of colorectal cancer prevention and control knowledge of a rural resident in Beijing under the “Chinese and Western medicine prevention and control model”, and to explore the relevant factors that may affect the awareness rate, in order to further optimize this model and to promote the prevention and treatment of colorectal cancer in Chinese rural areas and screening work to provide scientific basis. Methods Basic information collection, screening of high-risk groups, and core knowledge questionnaires on colorectal cancer prevention and treatment were conducted for villagers in a village in Beijing who participated in the “Beijing Traditional Chinese Medicine Healthy Village” pilot project from 2020 to 2021. Difference test and multivariate logistic regression were used to analyze the related factors affecting the awareness of core knowledge of colorectal cancer prevention and treatment. Results A total of 1007 valid questionnaires were recovered in this study and included in the analysis, with an effective recovery rate of 81.8%. The overall awareness rate of the core knowledge of colorectal cancer prevention and treatment among the surveyed population was 76.0%. There were significant differences in gender, age, education level, income level, and the awareness rate of people who had ever suffered from malignant tumor or had a family history of malignant tumor (P < 0.05). The results of multivariate logistic regression analysis showed that high educational level (secondary school/high school [OR = 2.124], college / university and above [OR = 2.852]), per capita annual income ≥ 20 000 yuan (OR = 4.204), the malignant tumor or family history of malignant tumor (OR = 4.839), and age 45-<60 years (OR = 3.262) were positive influencing factors for knowledge of colorectal cancer prevention and treatment (P < 0.05). Conclusion The awareness rate of colorectal cancer in a rural area in Beijing is relatively high, which meets the basic requirements of the state for residents’ knowledge of cancer prevention and control, while age, education level, per capita annual income, and whether one has ever suffered from malignant tumor or has a family history of malignant tumor are the main factors that affect residents’ awareness of cancer prevention.

Changes of drug resistance of Pseudomonas aeruginosa and rationality analysis of antibiotics in respiratory department of our hospital

Objective To investigate the drug resistance of Pseudomonas aeruginosa (PA) in respiratory department and the choice of antibiotics in patients to provide evidence for rational use of antibiotics. Methods The trend of drug resistance of PA in the Third Affiliated Hospital of Anhui Medical University (hereinafter referred to as “our hospital”) from 2013 to 2020 was retrospectively analyzed, and the antimicrobial resistance rate of patients with PA infection in respiratory department was retrospectively analyzed. According to the drug susceptibility results, the included patients were divided into carbapenem-resistant pseudomonas aeruginosa (CRPA) group (31 cases) and non-carbapenem-resistant pseudomonas aeruginosa (NCRPA) group (127 cases). The rationality and outcome of the choice of antibiotics in the two groups were evaluated. Results The drug resistance of PA to ceftazidime, cefepime, piperacillin-tazobactam, and imipenem were showed a trend of first decline and then increased, and the drug resistance rate was slightly higher than that of the whole hospital. The mortality rate in the CRPA group was higher than that in the NCRPA group, while the rational rate of antibiotic selection was lower than that in the NCRPA group, with statistically significant differences (P < 0.05). The unreasonable situations were that the combination of antibiotics was not used according to the drug susceptibility results, and the PA-sensitive antibiotics were not selected. Conclusion After 2015, the PA drug resistance rate has been increasing year by year. Paying attention to patients with high risk factors of drug resistance and selecting antimicrobial drugs based on drug susceptibility results can guide the treatment of PA.

Satisfaction survey on standardized training of professional degree postgraduates in a hospital in Beijing

Objective To investigate the status of satisfaction of professional degree graduate students participating in the standardized training of resident physicians, and to provide reference for improving the satisfaction and quality of standardized training. Methods From September 2018 to June 2021, a cluster survey was used to conduct a questionnaire survey on 281 professional degree graduate students who participated in the standardized training of residents in Beijing Friendship Hospital, Capital Medical University. The questionnaire consisted of a general information questionnaire and a training satisfaction questionnaire. The satisfaction questionnaire included six dimensions and was designed in the form of Likert scale. The satisfaction scores and the differences in satisfaction scores of graduate students with different characteristics were analyzed. Results A total of 281 questionnaires were distributed in this survey, 281 valid questionnaires were recovered, and the effective recovery rate was 100%. The overall satisfaction score of postgraduates to regular training was (3.78±0.20) points. Among them, graduate students scored the highest points of(4.19±0.45) for training conditions, while scientific research training (3.57±0.41) points and standardized management (3.52±0.52) points were in the last two points of each dimension. There was a statistically significant difference in the scores of standardized management and tutor guidance of graduate students of different grades (P < 0.05). There were statistically significant differences in the scores of scientific research training and training effect of postgraduates in different disciplines (P < 0.05). Conclusion The overall satisfaction of professional degree postgraduates with the regulation training is at a medium level, and it is suggested to improve the regulation training system, strengthen the implementation of the system, and play the guiding role of the tutor, so as to improve the satisfaction and quality of the regulation training.

Prediction value of QRS-T angle of electrocardiogram combined with admission hypersensitive C-reactive protein in patients with coronary heart disease complicated with malignant arrhythmia after percutaneous coronary intervention

Objective To explore the prediction value of QRS-T angle of electrocardiogram combined with admission hypersensitive C-reactive protein (hs-CRP) in patients with coronary heart disease complicated with malignant arrhythmia after percutaneous coronary intervention. Methods A total of 72 patients with coronary heart disease admitted to Rugao Hospital of Traditional Chinese Medicine, Jiangsu Province from August 2016 to July 2021 were selected as the study subjects. According to whether the patients were complicated with malignant arrhythmia within one month after percutaneous coronary intervention, they were divided into combined group (27 cases) and uncombined group (45 cases). The medical records, hematological indexes, and QRS-T angle of electrocardiogram after admission were compared between the two groups, the related influencing factors postoperative malignant arrhythmia in patients with coronary heart disease after percutaneous coronary were analyzed, and the predictive value of QRS-T angle of electrocardiogram and admission hs-CRP level for postoperative malignant arrhythmia in patients with coronary heart disease were evaluated. Results The proportion of smoking history, proportion of blood potassium level < 3.5 mmol/L, QRS-T angle, and admission hs-CRP level in the combined group were higher than those in the uncombined group, the differences were statistically significant (P < 0.05). Multivariate results showed that smoking history, blood potassium level < 3.5 mmol/L, QRS-T angle of electrocardiogram, and admission hs-CRP level were the influencing factors of postoperative malignant arrhythmia in patients with coronary heart disease after percutaneous coronary intervention (P < 0.05). The predictive value of QRS-T angle combined with admission hs-CRP level in patients with coronary heart disease after percutaneous coronary intervention was higher than that of the two alone (P < 0.05). Conclusion QRS-T angle of electrocardiogram combined with admission hs-CRP level has high predictive efficacy in patients with coronary heart disease after percutaneous coronary intervention complicated with malignant arrhythmia, which is worthy of further promotion.

Predictive value of peripheral blood neutrophil / lymphocyte ratio combined with C-reactive protein in major adverse cardiac events after percutaneous coronary intervention in patients with acute ST segment elevation myocardial infarction

Objective To investigate the predictive value of peripheral blood neutrophil / lymphocyte ratio (NLR) combined with C-reactive protein (CRP) in major adverse cardiac events after percutaneous coronary intervention (PCI) in patients with acute ST segment elevation myocardial infarction (STEMI). Methods Eighty patients with acute STEMI treated with PCI from June 2017 to June 2020 in Wuhu Second People’s Hospital, Anhui Province, were divided into the occurrence group (13 cases) and the non-occurrence group (67 cases) according to their incidence of major adverse cardiac events in one year after surgery. The clinical data of the two groups were collected and compared to analyze the influencing factors of major adverse cardiac events in acute STEMI patients after PCI and the predictive value of NLR and CRP on major adverse cardiac events. Results Serum uric acid (UA), serum phosphorus (P), triglyceride (TG), NLR, and CRP in the occurrence group were higher than those in the non-occurrence group, and the differences were statistically significant (P < 0.05). Multivariate analysis showed that UA, P, TG, NLR, and CRP were the influencing factors of major adverse cardiac events after PCI in acute STEMI patients (P < 0.05). The area under the receiver operation characteristic curve of peripheral blood NLR combined with CRP predicted major adverse cardiac events in acute STEMI patients after PCI was higher than those predicted by the them alone (P < 0.05). Conclusion Both NLR and CRP in peripheral blood are associated with the occurrence of major adverse cardiac events in acute STEMI patients after PCI, and both have good predictive value, and combined detection has better predictive efficacy.

Visual analysis of domestic literature on community intervention for stroke patients in China

Objective To visually present the current status and development trend of community intervention for stroke patients in China in recent years, and to provide further thinking and reference for researchers in related fields. Methods Based on CNKI database, the published literature on community intervention for stroke patients from January 2000 to July 2021 was retrieved, while the authors, publishing institutions, keywords, and emerging words were visually analyzed through CiteSpace software. Results The 585 articles were finally included in the analysis. The research strength of domestic community health service centers and other grass-roots institutions was generally weak, and the authors and core authors with high frequency were relatively limited, while research hotspots havd become mainstream. Recent fronts had focused on the elderly, quality of life, community management, and home care. Conclusion Community intervention research for stroke patients in my country should further focus on the home care needs of stroke patients, and explore the degree of connection and interaction between stroke patients, family caregivers and community health service centers, so as to provide a basis for optimizing the whole-course stroke community intervention.

Clinical effect of repetitive transcranial magnetic stimulation combined with Fluoxetine in the treatment of post-stroke depression

Objective To observe the clinical effect of different modes of repetitive transcranial magnetic stimulation (rTMS) combined with Fluoxetine in the treatment of post-stroke depression (PSD). Methods A total of 60 PSD patients admitted to Beijing Boai Hospital of China Rehabilitation Research Center from January 2020 to November 2021 were selected and divided into low frequency treatment group, high frequency treatment group, and control group according to the random number table method, with 20 cases in each group. rTMS at 1, 5 Hz, and sham stimulation were given for 14 days, respectively. The three groups were also given Fluoxetine 20 mg/d, conventional drug therapy and rehabilitation therapy for 28 days. Before treatment, on the 7th, 14th, and 28th days of treatment, the scores of Hamilton depression scale (HAMD), National Institute of Health stroke scale (NHISS), modified Barthel index (MBI), and mini-mental state examination (MMSE) and adverse reactions were compared among the three groups. Results The overall analysis showed that there were significant differences in HAMD score time, between groups, and interaction among the three groups (P < 0.05). Further pairwise comparison and intra-group comparison: the HAMD scores of the low and high frequency treatment groups on the 7th, 14th, and 28th days of treatment were lower than those before treatment in the same group, while the HAMD scores of the control group on the 14th and 28th days of treatment were lower than those before treatment in the same group, and the differences were statistically significant (P < 0.05). Comparison between groups: before treatment, there was no significant difference in HAMD scores between groups (P > 0.05). After treatment, the HAMD scores in the low and high frequency treatment groups were lower than those in the control group, and the differences were statistically significant (P < 0.05). On the 28th day of treatment, the NHISS scores of the three groups were lower than those before treatment, while the scores of MBI and MMSE were higher than those before treatment, and the differences were statistically significant (P < 0.05). On the 28th day of treatment, the differences in NHISS and MBI scores among the three groups were statistically significant (P < 0.05). Among them, the NHISS score of the low and high frequency treatment groups were lower than that of the control group, while the MBI score was higher than that of the control group, and the differences were statistically significant (P < 0.05). There were no adverse reactions in the control group and the low frequency treatment group during the treatment, and three patients in the high frequency treatment group developed mild to moderate transient headaches after treatment, which resolved spontaneously after rest. Conclusion rTMS combined with Fluoxetine therapy and Fluoxetine therapy alone can both improve mood, reduce neurological deficit, and improve activites of daily living and cognitive level in patients with PSD; combination therapy has a faster onset and more significant curative effect, and the curative effect is still sustainable after the magnetic stimulation is stopped, and the adverse reactions are small.

Effects of mild hypothermia combined with hyperbaric oxygen on cognitive function in patients with delayed encephalopathy due to carbon monoxide poisoning

Objective To observe the effects of mild hypothermia combined with hyperbaric oxygen therapy on neuron specific enolase (NSE), S100β, and cognitive function in patients with delayed encephalopathy due to carbon monoxide poisoning (DEACMP). Methods A total of 70 patients with DEACMP who were admitted to the Department of Emergency of the First Hospital of Hebei Medical University from October 2018 to December 2020 were selected and divided into observation group and control group according to the random number table method, with 35 cases in each group. Both groups were given neurotrophic, free radical scavenging, and symptomatic treatment, the control group was given hyperbaric oxygen therapy, while the observation group was given mild hypothermia + hyperbaric oxygen therapy. The curative effect of the two groups was evaluated, and the changes of NSE, S100β levels, cognitive function, and daily living ability were observed before and after treatment in the two groups. Results After three months of treatment, the total effective rate of the observation group was significantly higher than that of the control group, and the difference was statistically significant (P < 0.05). After treatment, NSE and S100β in the two groups were lower than those before treatment, while those of the observation group were lower than those of the control group, and the differences were statistically significant (P < 0.05). The scores of the mini-mental state examination scale and the abilities of daily living scale in the two groups after treatment were higher than those before treatment, those of the observation group were higher than the control group, and the differences were statistically significant (P < 0.05). Conclusion Mild hypothermia combined with hyperbaric oxygen therapy for DEACMP can effectively protect brain cell damage, improve patients’ cognitive function and daily living ability, enhance their quality of life, promote patients’ recovery, and improve prognosis, which is worthy of clinical application.

Systematic evaluation of decision aids in patients with first stroke

Objective To systematically evaluate the effect of decision aids in patients with first stroke. Methods Literatures on the application of decision aids in first stroke patients from the inception to June, 2021 of the Cochrane Library, PubMed, Embase, CINAHL, CBM, CNKI, and Wanfang Data were searched. The literatures were screened and evaluated for quality. RevMan 5.3 software was used for meta-analysis. Results A total of five articles were included. There were no significant differences in mortality (RR = 0.61, 95%CI: 0.20-1.88, P = 0.39) and incidence of cerebral hemorrhage after thrombolysis (RR = 0.86, 95%CI: 0.31-2.41, P = 0.77) between the two groups. The 60 min thrombolytic rate (RR = 1.44, 95%CI: 1.16-1.78, P = 0.001) and thrombolytic decision agreement rate (RR = 1.12, 95%CI: 1.01-1.25, P = 0.03) in the experimental group were higher than those in the control group, and the differences were statistically significant. Conclusion Decision aids in patients with first stroke can improve the 60 min thrombolytic rate and thrombolytic decision agreement rate, and it is safe, which is worthy of further clinical research and promotion.

Changes and clinical value of serum inflammatory factors in patients with enlarged perivascular space in basal ganglia region

Objective To explore the changes and clinical value of serum inflammatory factors in patients with enlarged perivascular space (EPVS) in basal ganglia region. Methods A total of 60 patients admitted to the Department of Neurology, the First Affiliated Hospital of Hebei North University (hereinafter referred to as “our hospital”) from March 2018 to March 2019 with EPVS detected by cranial magnetic resonance imaging were selected as the EPVS group, and 30 healthy subjects in our hospital during the same period were selected as the control group. The serum levels of interleukin (IL)-1, IL-6, tumor necrosis factor-α (TNF-α), and C-reactive protein (CRP) were compared between the two groups. The differences of IL-1, IL-6, TNF-α, and CRP in patients with different EPVS grades were further compared. The diagnostic value of IL-1, IL-6, TNF-α, and CRP for EPVS in basal ganglia region was analyzed by receiver operating characteristic curve and area under curve (AUC). Results Serum IL-1, IL-6, TNF-α, and CRP levels in EPVS group were higher than those in control group, the differences were statistically significant (P < 0.05). The serum levels of IL-1, IL-6, TNF-α, and CRP in patients with grade two and three EPVS in basal ganglia region were higher than those in patients with grade one EPVS in basal ganglia region, and the serum levels of IL-1, IL-6, TNF-α, and CRP in patients with grade three EPVS in basal ganglia region were higher than those in patients with grade two EPVS in basal ganglia region, the differences were statistically significant (P < 0.05). Serum IL-1, IL-6, TNF-α, and CRP had diagnostic value for EPVS in basal ganglia region alone or in combination datection (AUC > 0.05, P < 0.05). Conclusion The serum levels of IL-1, IL-6, TNF-α, and CRP in patients with EPVS in the basal ganglia region are abnormally elevated. Combined detection of the above serological indicators could help improve the diagnosis rate of EPVS in the basal ganglia region and provide scientific reference for clinical treatment.

Meta-analysis of influencing factors for malignant cerebral edema in patients with acute cerebral infarction

Objective To systematically review the influencing factors of malignant cerebral edema in patients with acute cerebral infarction. Methods PubMed, Web of Science, EMbase, the Cochrane Library, CNKI, VIP, and Wanfang Data were searched to collect relevant literatures on influencing factors of malignant cerebral edema in patients with acute cerebral infarction. The retrieval period is from the inception to April 2021. Two reviewers independently screened literatures, extracted data, and assessed the quality of included studies, then, meta analysis was performed by RevMan 5.3 and Stata 15.0 software. Results A total of 18 literatures were included, including 66 792 patients. Meta-analysis results showed baseline National Institute of Health stroke scale (NIHSS) score (OR = 1.15, 95%CI: 1.15-1.16, P < 0.01), history of atrial fibrillation (OR = 2.99, 95%CI:1.53-5.87, P < 0.01), anterior cerebral artery involvement (OR = 5.53, 95%CI: 2.48-12.34, P < 0.01), carotid artery occlusion (OR = 2.12, 95%CI:1.04-4.32, P = 0.04) were the risk factor for malignant cerebral edema after cerebral infarction. Sensitivity analysis showed that successful recanalization was a protective factor (OR = 0.36, 95%CI: 0.20-0.66, P < 0.01), the stability was changed. Egger test results showed no publication bias (P > 0.05). Conclusion Baseline NIHSS score, history of atrial fibrillation, anterior cerebral artery involvement, and carotid artery occlusion are risk factors for malignant cerebral edema after cerebral infarction, which should be paid attention to and timely take corresponding measures in clinical practice. The relationship between successful recanalization and malignant cerebral edema still needs further study.

[Abstract] Objective To preliminarily analyse the serum levels of vitamin A, 25-hydroxy-vitamin D3, and vitamin E in children with primary development delay (DD) and their correlation with neurodevelopment. Methods Thirty-three children with primary DD who were treated in the Department of Neurology, Children’s Hospital, Capital Institute of Pediatrics (hereinafter referred to as “our hospital”) from March 2019 to February 2020 were selected as primary DD group, and 30 children who underwent routine physical examinations in the Department of Child Health during the same period served as healthy control group. The serum level of vitamin A, 25-hydroxy-vitamin D3, and vitamin E of two groups were measuerd by high performance liquid chromatography tandem mass spectrometry. The neurodevelopmental level of children with primary DD was evaluated by developmental scale for children aged 0-6 years, and the correlation between the scores of each functional area and vitamin content was analyzed. Results The serum level of vitamin A and 25-hydroxy-vitamin D3 in primary DD group were lower than those in healthy control group (P < 0.05). There was no significant difference in serum vitamin E content between two groups (P > 0.05). The deficiency rates of vitamin A, 25-hydroxyvitamin D3, and vitamin E in primary DD group were higher than those in healthy control group (P < 0.05). The level of 25-hydroxy-vitamin D3 was positively correlated with the score in the fine motion dimension (r = 0.375, P < 0.05). Vitamin A and 25-hydroxy-vitamin D3 were protective factors for primary DD (OR < 1, P < 0.05). Conclusion The serum levels of vitamin A and 25-hydroxy-vitamin D3 in primary DD children are significantly lower than those of healthy children of the same age, serum 25-hydroxy-vitamin D3 deficiency affects fine motor development in children.

Effect of Huayu Xiaozheng Decoction retention enema on endometriosis and its effect on recurrence rate

Objective To explore the effect of Huayu Xiaozheng Decoction retention enema on endometriosis and its effect on recurrence rate. Methods From July 2018 to September 2020, 76 patients with endometriosis who were admitted to Baoshan Hospital of Integrated Traditional Chinese and Western Medicine were selected, according to the random number table method, they were divided into the control group (38 cases) and the observation group (38 cases). Both groups underwent minimally invasive surgery, the control group was treated with Gestrinone on this basis, and the observation group was treated with Huayu Xiaozheng Decoction retention enema on this basis. The therapeutic effect, traditional Chinese medicine syndrome scores, sex hormone levels, serum indexes, and recurrence rates were compared between the two groups. Results The clinical effect of the observation group was higher than that of the control group (P < 0.05). Six months after treatment, the scores of dysmenorrhea, breast tenderness, and anal bulge in the two groups were lower than those before treatment, and the observation group was lower than the control group (P < 0.05); the follicle-stimulating hormone and estradiol levels in the two groups were lower than those before treatment, and the observation group was lower than the control group (P < 0.05); carbohydrate antigen 125 (CA125), CA199, and endometrial antibodies in the two groups were lower than those before treatment, and the observation group was lower than the control group (P < 0.05). The recurrence rate of the observation group was lower than that of the control group (P < 0.05). Conclusion The use of Huayu Xiaozheng Decoction retention enema after minimally invasive surgery for patients with endometriosis has great significance in reducing the recurrence rate of the disease and promoting blood circulation. It can not only reduce the inflammatory response of patients, improve the pregnancy rate, but also inhibit the growth of ectopic endometriosis and improve the ovarian function of patients.

Relationship between urinary FSP-1, β2-MG and severity of henoch-schonlein purpura in children and prognosis factors analysis

Objective To investigate the severity of urinary fibroblast marker protein-1 (FSP-1), β2-microglobulin (β2-MG) of children with henoch-schonlein purpura (HSP) and the factors affecting prognosis. Methods From January 2018 to December 2020, 110 children with HSP admitted to the Department of Pediatrics, the First Affiliated Hospital of Anhui University of Science and Technology (“our hospital” for short) were selected as HSP group. During the same period, 100 healthy children who came to our hospital for health consultation were control group. The levels of urinary FSP-1 and β2-MG were compared between two groups, and the correlation between the levels and purpura symptoms scores was analyzed. The children were divided into kidney injury group and non-injury group according to whether kidney injury occurred within one week after admission, and the risk factors influencing poor prognosis of HSP were analyzed. Results The levels of urinary FSP-1 and β2-MG in HSP group were higher than those in control group, and the differences were statistically significant (P < 0.05). Pearson correlation analysis showed that urinary FSP-1 and β2-MG levels were positively correlated with purpura symptom scores (r > 0, P < 0.05). There were 35 cases of renal injury in 110 cases, and there were statistically significant differences in abdominal pain, IgA, urinary FSP-1, urinary β2-MG, and purpura symptom scores between two groups (P < 0.05). Logistic regression analysis showed that abdominal pain, IgA, urinary FSP-1, β2-MG, and purpura symptom score were the effect factors for poor prognosis (OR > 1, P < 0.05). Conclusion Abnormal levels of urinary FSP-1 and β2-MG in children with HSP and positively correlated with the severity of the disease, which are risk factors leading to poor prognosis.

Meta-analysis of Bevacizumab combined with Cisplatin in the treatment of malignant pleural effusion of lung cancer

Objective To evaluate the clinical efficacy of Bevacizumab combined with Cisplatin in the treatment of malignant pleural effusion of lung cancer. Methods The clinical data were collected from Chinese Journal full-text Database, Wanfang Data, VIP, Chinese Biomedical Literature Service system, PubMed, Embase, Central and Medline from the establishment of the database to August 2021. Two systematic reviewers searched the clinical randomized controlled trial of Bevacizumab combined with Cisplatin in the treatment of malignant pleural effusion of lung cancer, and then screened the literatures, extracted the data in the literature, and evaluated the quality of the literatures. The data were analyzed by Revman 5.3 software. Results A total of 17 studies were included, with a total of 1028 patients. The results of meta-analysis showed that the clinical efficacy (OR = 4.02, 95%CI:2.97 to 5.43, P < 0.000 01) of experimental group was better than the control group, vascular endothelial growth permeability factor level in pleural effusion (MD = 53.24, 95%CI:-55.46 to -51.03, P < 0.000 01) in the experimental group were significantly higher than those in the control group, the difference was statistically significant. The gastrointestinal adverse reactions in the control group (OR = 0.64, 95%CI: 0.43 to 0.96, P = 0.03) were significantly higher than those in the experimental group, the difference was statistically significant. There was no significant difference in serum carcinoembryonic antigen level, overall adverse reactions, bone marrow suppression and liver and kidney function injury between the two groups (P > 0.05). Conclusion Bevacizumab combined with Cisplatin pleural perfusion is more effective in the treatment of malignant pleural effusion of lung cancer. As the evaluation of the clinical literature quality of this study is not high, it is necessary to carry out more meticulous, detailed, high-quality, and randomized double-blind and multicenter clinical controlled trials to more effectively demonstrate its clinical efficacy in the treatment of malignant pleural effusion of lung cancer.

Expression and clinical significance of serum miR-125b and miR-18b in patients with colon cancer

Objective To investigate the expression and clinical significance of serum miR-125b and miR-18b in patients with colon cancer. Methods A total of 134 patients with colon cancer treated in Shijiazhuang People’s Hospital (“our hospital” for short) from January 2015 to August 2016 were selected as the colon cancer group, Another 70 healthy subjects in our hospital were selected as the control group. The relative expressions of serum miR-125b and miR-18b were detected by qRT-PCR; the relationship between the expression of serum miR-125b, miR-18b and the clinicopathological characteristics was analyzed; the survival curves of colon cancer patients with different serum miR-125b and miR-18b expressions were plotted by K-M method; receiver operating characteristic curve analysis was used to analyze the predictive value of serum miR-125b and miR-18b expression on the poor prognosis of colon cancer. Results The expression of serum miR-125b in the colon cancer group was lower than that in the control group, and the expression of miR-18b was higher than that in the control group (P < 0.05). There were significant differences in the expression of serum miR-125b and miR-18b in patients with different TNM stages and lymph node metastasis (P < 0.05). The overall survival rate of miR-125b high expression group was higher than that of miR-125b low expression group; the overall survival rate of miR-18b high expression group was lower than that of miR-18b low expression group (P < 0.05). Area under the curve of miR-125b combined with miR-18b in predicting poor prognosis of colon cancer was greater than that of miR-125b and miR-18b alone predicted (P < 0.05). Conclusion The decrease of serum miR-125b expression and the increase of miR-18b expression may be closely related to the occurrence and development of colon cancer, which can be used as a clinical reference index to judge the biological behavior and prognosis of colon cancer.

Methods A total of 102 cases of driver-gene positive advanced NSCLC patients admitted to the Fifth People’s Hospital of Wuhu, Anhui Province were selected as the research objects from December 2017 to December 2019. They were divided into control group (51 cases) and study group (51 cases) according to the random number table method. The control group was given conventional treatment plan combined with Gefitinib, and the study group was given Apatinib treatment on the basis of the control group. The anti-tumor efficacy and the levels of tumor markers before and after treatment between two groups were compared; adverse reactions between two groups were observed; the two groups were followed up for 12 months to calculate the overall survival rate. Results Clinical control rate of study group was higher than that of control group, and the difference was statistically significant (P < 0.05); there was no significant difference in objective remission rate between two groups (P > 0.05). After treatment, the levels of carcinoembryonic antigen, CYFRA21-1, and carbohydrate antigen 125 in two groups were lower than those before treatment, and study group was lower than control group, and the differences were statistically significant (P < 0.05). The total incidence of hypertension and proteinuria in study group was higher than that of control group, and the difference was statistically significant (P < 0.05); there was no significant difference in the total incidence of bone marrow suppression and hand-foot syndrome between two groups (P > 0.05). The overall survival rate of study group was higher than that of control group, and the difference was statistically significant (P < 0.05). Conclusion Apatinib combined with Gefitinib regimen in the treatment of driver-gene positive advanced NSCLC can enhance the anti-tumor efficacy, reduce the content of tumor markers, prolong the short-term survival rate of patients and have good safety.

Prognostic factors of neuronavigation combined with fluorescein sodium guided microsurgery in adult patients with high-grade gliomas

Objective To investigate the prognostic factors of neuronavigation combined with fluorescein sodium guided microsurgery in the treatment of adult patients with high-grade gliomas. Methods The clinical data of 35 adult patients with high-grade gliomas treated in the Department of Neurosurgery of the Inner Mongolia People’s Hospital from March 2016 to October 2020 were analyzed retrospectively. All patients underwent glioma resection with Brain lab neuronavigation system combined with fluorescein sodium (1.5-2.0 mg/kg) guided intraoperative adjuvant technology. Prognostic analysis of clinical factors affecting postoperative survival time was carried out. Results There were 14 cases of world health organization grade Ⅲ and 21 cases of world health organization grade Ⅳ. Total tumor resection was performed in 24 cases, and partial tumor resection in 11 cases. The follow-up time was 6-60 months, with 22 cases recurred and 20 cases died. The 1-year, 2-year, and 3-year survival rates were 76.4%, 44.4%, and 23.8%, respectively. There were statistically significant differences in survival rate among patients with different gender, tumor site, resection degree, pathological grade, Ki-67 index range, treatment mode, with or without epilepsy, and with or without complications (P < 0.05). Postoperative complications and Ki-67 positive indicators were independent risk factors affecting prognosis of patients (RR = 7.406, P = 0.026; RR = 3.524, P = 0.028). Conclusion Postoperative with or without complications, Ki-67 positive expression level, and the tumor site can be used as important factors to assess the prognosis.

Expression of lncRNA UCA1, miR-590-3p in gastric cancer tissues and the relationship with pathological parameters and prognosis

Objective To investigate the expression of long non-coding RNA (lncRNA) urothelial cancer associated 1 (UCA1) and microRNA (miR)-590-3p in gastric cancer tissues and the relationship with pathological parameters and prognosis. Methods A total of 107 case of gastric cancer patients admitted to Tangshan Hospital of Traditional Chinese Medicine of Hebei Province from January 2017 to July 2018 were selected, and lncRNA UCA1, miR-590-3p expression were detected in cancer and cancer-adjacent tissues by qRT-PCR. The correlation between lncRNA UCA1 and miR-590-3p expression in gastric cancer tissues and the relationship with pathological parameters were analysed. Survival curves of patients with different lncRNA UCA1, miR-590-3p expression in gastric cancer were plotted using the K-M method. Results The expression of lncRNA UCA1 in gastric cancer tissues was higher than that in cancer-adjacent tissues, and the expression of miR-590-3p was lower than that in cancer-adjacent tissues (P < 0.05). Pearson correlation analysis showed that lncRNA UCA1 was negatively correlated with miR-590-3p expression in gastric cancer tissues (r = -0.720, P < 0.01). The expression of lncRNA UCA1 and miR-590-3p in patients with different clinicopathological stages and with or without lymph node metastasis was significantly different (P < 0.05). K-M survival curve showed that the cumulative survival rate of high lncRNA UCA1 expression group was lower than that of low lncRNA UCA1 expression group, and the cumulative survival rate of high miR-590-3p expression group was higher than that of low miR-590-3p expression group (χ2=8.140, 11.624; P = 0.004, 0.001). Conclusion High expression of lncRNA UCA1 and low expression of miR-590-3p in gastric cancer tissues are associated with clinicopathological stages, lymph node metastasis, and prognosis, and may be used as indicators for prognostic assessment.

Safety and prognosis observation of patients with different operation approaches for the treatment of differentiated thyroid cancer by radical thyroidectomy

Objective To observe the safety and prognosis of patients with differentiated thyroid cancer treated by radical thyroidectomy with different operation approaches. Methods Eighty-four patients with differentiated thyroid cancer admitted to Rugao Hospital Affiliated to Nantong University from January 2018 to December 2020 were selected, they were divided into control group and observation group by random number table method, with 42 cases in each group. The observation group was treated with modified Miccoli operation, and the control group was treated with radical thyroidectomy via areola approach. Perioperative indicators, visual analogue scale (VAS), European Organization for Research and treatment of cancer quality of life questionnaire (EORTC QLQ-C30) score, and postoperative complications were recorded. Results The operation time, hospital stay time, and neck recovery time in the observation group were shorter than those in the control group, and the intraoperative blood loss and postoperative drainage volume were smaller than those in the control group, with statistically significant differences (P < 0.05). Overall analysis showed that VAS score of two groups had statistical significance in time, between groups, and interaction (P < 0.05). Further pairwise comparison, intra-group comparison: VAS scores of both groups at 72 h after surgery were higher than those before surgery and 12, 24, and 48 h after surgery, with statistically significant differences (P < 0.05). Comparison between groups: VAS score of the observation group was lower than that of the control group at 12 and 24 h after surgery, and the differences were statistically significant (P < 0.05). One month after surgery, QLQ-C30 scores in both groups were higher than those before surgery, and physical function, emotional function, social function, and overall health scores in observation group were higher than those in control group, and the differences were statistically significant (P < 0.05). There was no significant difference in the total incidence of postoperative complications between the two groups (P > 0.05). Conclusion Modified Miccoli operation treatment for differentiated thyroid cancer patients can reduce intraoperative blood loss and postoperative drainage volume, reduce patients’ recovery time and hospital stay time, improve patients’ prognosis, and have high safety, which is worthy of promotion.

The estimated value of melanoma-associated antigen A10 for the postoperative recurrence of non-muscular invasive bladder cancer

Objective To explore the estimated value of melanoma-associated antigen A10 (MAGE-A10) in the postoperative recurrence of patients with non-muscular invasive bladder cancer (NMIBC) after transurethral resection of bladder tumor (TURBt). Methods The age, gender, MAGE-A10 positive expression, tumor size, tumor grade, tumor stage, with or wthout immediate bladder perfusion, with or wthout postoperative tumor recurrence, and recurrence time of 106 patients with NMIBC from March 2016 to June 2020 in Suqian Hospital Affiliated to Xuzhou Medical University were retrospectively analyzed, the independent risk factors for postoperative recurrence were analyzed. Results Among 106 patients, 41 patients (38.68%) were MAGE-A10 positive. The patients were followed up for 12~51 months after operation, of which 46 cases (43.40%) recurred. The number of MAGE-A10 positive, tumor number, tumor size, tumor grade, and tumor stage had significant influence on postoperative tumor recurrence (P < 0.01). Cox univariate and multivariate regression analysis were found that MAGE-A10 positive, high tumor grade, and high tumor stage were independent risk factors for postoperative tumor recurrence (P < 0.05). There was significant difference in the median recurrence free survival time between MAGE-A10 positive and negative groups (P < 0.01). The area under the curve of receiver operator characteristic curve for predicting postoperative recurrence of patients with MAGE-A10 was 0.773 (P < 0.05). Conclusion The positive expression of MAGE-A10 is a related risk factor for the recurrence of the patients of non-muscular invasive bladder cancer, and it might be considered as a new potential indicator for evaluating the recurrence of the patients of non-muscular invasive bladder cancer.

Expression of CXCR7, CXCL11 and CXCL12 biological axis in the peripheral blood of patients with lupus nephritis

Objective To investigate the expression level and significance of CXC chemokine receptor 7 (CXCR7) and CXC-type chemokine ligand (CXCL) 11 and CXCL12 in peripheral blood of lupus nephritis (LN). Methods Forty patients with systemic lupus erythematosus (SLE) treated in the Department of Rheumatology, Xuzhou Central Hospital(“our hospital” for short) from October 2018 to October 2020 were selected as the study subjects. SLE patients were divided into the LN group (n 24 cases) and the non-LN group (16 cases). Twenty healthy subjects in the control group were examined in our hospital during the same period. Flow cytometry was used to detect the expression of CD3+CXCR7+T lymphocytes and CD19+CXCR7+B lymphocytes in peripheral blood of the three groups. The concentrations of CXCL11 and CXCL12 in serum were detected by enzyme-linked immunosorbent assay. Results The expression percentage of CD3+CXCR7+T lymphocyte and CD19+CXCR7+B lymphocyte in peripheral blood and the expression levels of CXCL11 and CXCL12 in serum of LN group were higher than those of non-LN group and healthy control group, the differences were statistically significant (P < 0.05). The expression levels of CD3+CXCR7+T lymphocytes, CD19+CXCR7+B lymphocytes, CXCL11 and CXCL12 in the non-LN group were higher than those of healthy control group, the differences were statistically significant(P < 0.05). The expression levels of CD19+CXCR7+ lymphocytes, CD3+CXCR7+ lymphocytes, CXCL11, and CXCL12 in peripheral blood of patients with LN were positively correlated with the quantification of 24 h urinary protein (P < 0.001). Conclusion The high expression levels of CXCR7, CXCL11, and CXCL12 are related to the pathogenesis of LN patients. The CXCR7/CXCL11/CXCL12 may play an important role in the pathogenesis of LN.

Effect of isokinetic strength training of thigh muscles on knee joint function and posture control after anterior cruciate ligament reconstruction

Objective To investigate the effect of isokinetic strength training of thigh muscles on knee joint function and posture control after anterior cruciate ligament (ACL) reconstruction. Methods Eighty-six patients undergoing ACL reconstruction admitted to the First People’s Hospital of Lianyungang from March 2017 to February 2020 were selected as the research objects, and they were divided into two groups according to the random number table method, with 43 cases in each group. The control group was given conventional rehabilitation training after the operation, and the study group was given isokinetic training of the thigh muscles after the operation on the basis of the control group. Follow-up for one year, three months after operation (T0), six months after operation (T1), and 12 months after operation (T2), the peak extension torque (PT), knee function, and posture control ability of the two groups were compared. Results There were statistically significant differences betwween the two groups in knee extension PT, Lysholm score, eye-closed foam plane gravity swing speed, time point comparison and interaction between the two groups (P < 0.05). At T1, knee extension PT, Lysholm score, and eye-closed foam plane gravity swing speed in the two groups were higher than T0, and the differences were statistically significant (P < 0.05), at T2, the knee extension PT, Lysholm score, and eye-closed foam plane gravity swing speed between the two groups were higher than those at T0 and T1, and the differences were statistically significant (P < 0.05). At T0, there were no significant differences between the two groups in terms of knee extension PT, Lysholm score, and eye-closed foam plane gravity swing speed (P > 0.05), at T1 and T2, the knee extension PT, Lysholm score, and eye-closed foam plane gravity swing speed in the study group were higher than those in the control group, at the some time point, and the differences were statistically significant (P < 0.05). Conclusion Isokinetic training of the thigh muscles can improve the knee joint function and posture control ability of patients after ACL reconstruction.

Relationship between leukocyte differentiation antigen 40 ligand G gene polymorphism and the correlation and prognosis of cirrhosis after chronic hepatitis B in the elderly

Objective To investigate the relationship between the G gene polymorphism of leukocyte differentiation antigen 40 ligand (CD40L) and the prognosis of cirrhosis after chronic hepatitis B in the elderly. Methods A total of 82 elderly patients with liver cirrhosis after chronic hepatitis B who were admitted to NO.904 Hospital of the Joint Logistics Support Force of the Chinese People’s Liberation Army (“our hospital” for short) from May 2016 to March 2019 were selected as the study group, and 80 healthy people who underwent physical examination in our hospital during the same period were selected as the control group. Peripheral venous blood was collected and blood genomic DNA was extracted, the CD40L gene polymorphism was determined, and its genotype distribution was determined. Results The CD40L genotypes of the two groups conformed to Hardy-Weinberg equilibrium law. The G allele frequency of the study group was higher than that of the control group (P < 0.05). The logistic regression model analysis showed that the relative risk of poor prognosis in elderly patients with CG and GG genotypes after chronic hepatitis B was 1.809 times and 2.264 times that of the CC genotype (P < 0.05) . At the end of the one year follow-up, the survival rate of elderly patients with CD40L G gene with liver cirrhosis after chronic hepatitis B was 62.90%. The survival rate of CD40L non-G gene cirrhosis after chronic hepatitis B was 85.00%. There was a statistically significant difference in the mortality curve of CD40L G gene and non-G gene patients (χ2 = 6.138,P < 0.05). Conclusion The frequency of the G allele in elderly patients with post-chronic hepatitis B liver cirrhosis is higher than that in healthy people. The CD40L gene polymorphism is related to the prognosis of death in patients with chronic hepatitis B after cirrhosis. The GG and CG genotypes can increase the risk of death.

Clinical effect of narrow-band intense pulsed light combined with Timolol in the treatment of patients with mild to moderate acne

Objective To explore the clinical effect of narrow-band intense pulsed light (DPL) combined with Timolol in the treatment of patients with mild to moderate acne. Methods A total of 102 patients with mild to moderate acne admitted to the Nanjing Hospital Affiliated to Nanjing University of Chinese Medicinel from January 2019 to May 2021 were selected as the research objects, and they were divided into combination group (51 cases) and control group (51 cases) according to the random number table method. The control group were treated with DPL, and the combination group were treated with Timolol on the basis of the control group. The clinical efficacy of the two groups after 12 weeky of treatments was compared, the acne-specific quality of life questionnaire (Acne-QOL), serum interleukin 1α (IL-1α) levels were compared between the two groups before and after treatment. The incidence of adverse reactions during treatment in the two groups was recorded. Results The clinical efficacy of the combination group was better than that of the control group, and the difference was statistically significant (P < 0.05). Before treatment, there was no significant difference in Acne-QOL scores between the two groups (P > 0.05), after treatment, the Acne-QOL scores of the two groups were higher than those before treatment, and the combination group was higher than the control group, the differences were statistically significant (P < 0.05). Before treatment, there was no significant difference in serum IL-1α levels between the two groups (P > 0.05), after treatment, serum IL-1α levels in the two groups were lower than those before treatment, and the combination group was lower than the control group, the differences were statistically significant (P < 0.05). There was no significant difference in the total incidence of complications between the two groups (P > 0.05). Conclusion DPL combined with Timolol has a definite effect in the treatment of patients with mild to moderate acne. It can improve the quality of life of patients, reduce the inflammation of the body, and it is safe and reliable.

Prognostic value of inflammatory indexes, NLR, PLR, red blood cell distribution width level detection in prognosis of patients with acute exacerbation of chronic obstructive pulmonary disease and type Ⅱ respiratory failure

Objective To explore the inflammatory indicators and detection of neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and red blood cell distribution width (RDW) in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) complicated with type Ⅱ respiratory failure and clinical significance.
Methods A total of 216 patients with AECOPD and type Ⅱ respiratory failure admitted to the Pukou Branch of Jiangsu Province Hospital from June 2018 to May 2021 were selected as the research objects. The survival status of patients with AECOPD combined with type Ⅱ respiratory failure at 28 d after hospitalization was calculated. The general data of the patients in the death group and the survival group were compared. Logistic multivariate regression analysis of factors affecting the prognosis of patients. The value of inflammatory indicators, NLR, PLR, and RDW in predicting the prognosis of patients was analyzed. Results AECOPD patients with type Ⅱ respiratory failure during hospitalization death rate was 15.74%. Multivariate results showed that acute physiology and chronic health status scoring system Ⅱ score (OR = 2.787, 95%CI:1.246-5.213), C-reactive protein (CRP) (OR = 3.374, 95%CI:1.875-5.964), NLR (OR = 4.336, 95%CI:2.943-8.475), PLR (OR = 3.865, 95%CI:2.306-7.815), and RDW (OR = 2.751, 95%CI:1.028-3.957) were risk factors for death (P < 0.05). Receiver operating characteristic curve analysis showed that the area under the curve of CRP, NLR, PLR, and RDW for predicting death in AECOPD patients with type Ⅱ respiratory failure were 0.731 (95%CI:0.666-0.789), 0.796 (95%CI:0.736-0.848), 0.768 (95%CI:0.706~0.823), 0.808 (95%CI:0.749~0.858), respectively. Conclusion CRP, NLR, PLR, and RDW have high predictive power for the death of AECOPD patients with type Ⅱ respiratory failure.

Effect of Longsha Kaihe Liuqi acupuncture in the treatment of acute peripheral facial paralysis

Objective To explore the clinical effect of Longsha Kaihe Liuqi acupuncture in the treatment of peripheral facial paralysis. Methods A total of 60 patients with acute peripheral facial paralysis admitted to the outpatient and inpatient departments of the Department of Acupuncture and Moxibustion in Wuhu Hospital of Traditional Chinese Medicine from January 2020 to November 2021 were selected, they were divided into control group and treatment group by random number table method, with 30 cases in each group. On the basis of western medicine treatment, the control group was treated with conventional acupuncture, the treatment group were treated with Longsha Kaihe Liuqi acupuncture. After 15 d of continuous treatment, H-B facial nerve function grade, and Yang’s facial nerve paralysis grade of the two groups before and after treatment were compared and analyzed, and their clinical efficacy were evaluated. Results There was no significant difference in clinical efficacy between the two groups (P > 0.05). After treatment, the H-B facial nerve function grade in both groups were lower than those before treatment (P < 0.05), but there was no statistical significance in the H-B facial nerve function grade of the two groups (P > 0.05). After treatment, the grade of Yang’s facial nerve paralysis in both groups were lower than those before treatment (P < 0.05), but there was no statistical significance in the grade of Yang’s facial nerve palsysis between the two groups (P > 0.05). Conclusion Longsha Kaihe Liuqi acupuncture can significantly improve the clinical symptoms of patients with peripheral facial paralysis, with few points and simple operation, it can replace conventional acupuncture, and it is suitable for clinical promotion.

Clinical effect of Jin’s Three Needles combined with rehabilitation training in the treatment of increased muscle tone of the hind upper limbs after stroke

Objective To explore the clinical effect of Jin’s Three Needles combined with rehabilitation training in the treatment of patients with increased muscle tone of the hind upper limbs after stroke. Methods A study of 102 patients with increased muscle tone of the upper limbs after stroke admitted to Jianhu Hospital affiliated to Nantong University from March 2020 to March 2021 were selected as the research subjects. They were divided into two groups by random number table method. The control group (51 cases) was given rehabilitation training intervention, and the study group (51 cases) was combined with Jin’s Three Needles therapy on the basis of control group. Four weeks after treatment, Fngl-Meyer score and Barthel index score were used to evaluate the upper limb motor function and daily living activities of patients in the two groups, and the nerve reflex function of the affected side was detected, and adverse reactions were recorded. Results The total effective rate of study group was higher than that of control group (P < 0.05). The overall analysis showed that there were statistically significant differences between two groups, time points, and interaction of upper limb motor function score (P < 0.05). Further pairwise comparison, intra-group comparison: the upper limb motor function scores of two and four weeks after treatment were higher than those before treatment (P < 0.05); comparison between groups: the upper limb motor function score of the study group was higher than that of the control group at two and four weeks after treatment (P < 0.05). Four weeks after treatment, H/M of reflected wave of radial nerve, ulnar nerve, and median nerve in two groups were lower than those before treatment, and those in the study group were lower than those in the control group (P < 0.05). The overall analysis showed that there were statistically significant differences between two groups, time points, and interaction of activities of daily living (P < 0.05). Further pairwise comparison, intra-group comparison: activities of daily living scores of two and four weeks after treatment were higher than those before treatment (P < 0.05); comparison between groups: the activities of daily living scores of the study group was higher than that of the control group at two and four weeks after treatment (P < 0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Rehabilitation training combined with Jin’s Three Needles therapy for patients with increased muscle tone after stroke can help improve the patient’s upper limb motor function and activities of daily living, improve the clinical rehabilitation effect, and has good safety.

Clinical effect of compound Huoxueling Decoction in the treatment of deep venous thrombosis after femoral neck fracture

Objective To investigate the effect of compound Huoxueling Decoction in the treatment of deep venous thrombosis after femoral neck fracture. Methods A total of 82 patients with deep venous thrombosis after femoral neck fracture treated in Suining Municipal Hospital of Traditional Chinese Medicine of Sichuan Province from June 2019 to June 2021 were selected, and they were divided into control group and observation group according to random number table method, with 41 cases in each group. The control group was treated with conventional Western medicine, and the observation group was treated with compound Huoxueling Decoction on the basis of the control group. Both groups were treated for two weeks. The clinical efficacy was evaluated after treatment. The difference of thigh and calf circumference, hemorheology indexes, and inflammatory factor levels were compared between the two groups before and after treatment. Results The clinical effect of the observation group was better than the control group (P < 0.05). After treatment, the difference of thigh and calf circumference between the two groups was decreased compared with that before treatment, and those in the observation group were smaller than those in the control group (P < 0.05). The levels of plasma viscosity, whole blood hypershear viscosity, platelet adhesion rate, interleukin-6, and tumor necrosis factor-α in two groups were lower than those before treatment, and those in the observation group were lower than those in the control group (P < 0.05). The level of serum interleukin-10 in both groups was higher than that before treatment, and that in the observation group was higher than that in the control group (P < 0.05). Conclusion On the basis of conventional Western medicine treatment, the application of compound Huoxueling Decoction in the treatment of deep venous thrombosis after femoral neck fracture has a significant effect, which can significantly reduce symptoms, improve blood hypercoagulability and inhibit inflammatory reaction.

Pathogenesis and treatment of polycystic ovary syndrome based on Sisheng Xinyuan

The theory of “cycle flow of one qi” proposed in Sisheng Xinyuan can explain the growth process of follicles, and the loss of qi circulation can often lead to ovulation disorders. Summarizing the academic thought of Sisheng Xinyuan, the key pathogenesis of polycystic ovary syndrome is soil dampness and wood stagnation, which can have water-cold, fire reversal, and gold stagnation. The treatment of polycystic ovary syndrome should be based on tonifying soil and thinning wood. According to the clinical manifestations, the treatment of middle soil and four dimensions should be differentiated, supplemented by eliminating dampness and phlegm, activating qi, and reducing fire to restore the body and ovary local physiological state of “cycle flow of one qi”. The main theories of Sisheng Xinyuan provide new ideas for clinical prevention and treatment of polycystic ovary syndrome, which is worth further digging and learning.

A bibliometric and visual analysis of research on shared decision-making research of medical field in China based on CiteSpace

[Abstract] Objective To understand the research status and hotspots of shared decision-making in medical field in China, so as to provide reference for further research and application of shared decision-making in China. Methods In CNKI, Wanfang and VIP databases, keywords were retrieved by “shared decision” OR “joint decision” OR “shared decision”, and the retrieval time was from the database construction to October 2021. NoteExpress and CiteSpace software were used for bibliometric and visual analysis. Results A total of 357 articles were included. The number of published articles in the year was gradually increasing. Zhou Lingming and the National Engineering Laboratory of Internet Medical System and Application were the authors and institutions with the largest number of papers, and Zhang Mingming and the School of Nursing of Shanghai Jiao Tong University were the teams and institutions with the highest degree of cooperation. The journal with the largest number of articles was Medicine and Philosophy. The research focuses on influencing factors, review, chronic diseases such as breast cancer, decision participation, and tools, etc. Conclusion CiteSpace software is used to perform bibliometrics and visual analysis on the literatures related to share decision-making in the medical field of China, which objectively clarify the research status, hot spots and trends, and provid a direction for further enhancing the breadth and depth of clinical application of shared decision-making in the future.

Effect of hospital-community-family integrated model on health behavior and cardiac function in patients with chronic heart failure

Objective To explore the effect of hospital-community-family integrated model on health behavior and cardiac function in patients with chronic heart failure (CHF). Methods From January to December 2019, 81 CHF patients hospitalized in the Department of Cardiovascular Medicine of the Second Affiliated Hospital of Guangxi Medical University were selected as the research objects. According to the random number table method, they were divided into control group (40 cases) and observation group (41 cases). The control group adopted the traditional health education mode, and the observation group adopted the hospital-community-family ternary linkage mode. Health behavior score, self-management efficacy score, dyspnea index (mMRC), 6-minute walking distance (6MWT), left ventricular ejection fraction (LVEF), and cardiac index (CI) were compared between the two groups before and after intervention. Results After intervention, 6MWT, LVEF, and CI of the two groups were higher than before intervention, while mMRC was lower than before intervention, and 6MWT, LVEF, and CI of the observation group were higher than the control group, while mMRC was lower than the control group, the differences were statistically significant (P < 0.05); the total scores of self-actualization, nutrition and exercise in the control group were higher than those before the intervention, and the health behavior scores and total scores in the observation group were higher than those before the intervention, and the self-actualization, health responsibility, nutrition, exercise and total scores in the observation group were higher than those in the control group, and the differences were statistically significant (P < 0.05); after intervention, drug management, psychological/social adjustment management, symptom management, and total score in the control group were higher than before intervention, while the score and total score in the observation group were higher than before intervention, and the score of drug management, diet management, symptom management, and total score in the observation group were higher than the control group, and the differences were statistically significant (P < 0.05). Conclusion The management of hospital-community-family ternary linkage model can effectively improve the health behavior of CHF patients, improve the efficiency of self-management and improve the cardiac function of patients, which is worthy of clinical application.

Application effect of comfort nursing based on ERAS concept in perioperative period of children treated with fiberoptic bronchoscopy

Objective To study the effect of comfort nursing based on the enhanced recovery after surgery (ERAS) in the perioperative period of fiberoptic bronchoscopy for children. Methods A total of 115 children requiring fiberoptic bronchoscopy were selected from Anhui Hospital Affiliated Pediatric Hospital of Fudan University from November 2018 to October 2021. According to the random number table method, they were randomly divided into 55 cases in the routine group and 60 cases in the ERAS group. After operation, the nursing effect, rale disappearance time, cough relief time, hospital stay, temperature return to normal time, and the incidence of postoperative complications were recorded and compared between the two groups. Results The compliance in ERAS group was better than that in routine group (P < 0.05); the cough relief time, rale disappearance time, body temperature return to normal time, and hospital stay in ERAS group were shorter than those in routine group (P < 0.05). After operation, the incidence of complications in ERAS group was lower than that in routine group (P < 0.05). Conclusion The comfortable nursing under the guidance of ERAS concept in the perioperative period of children’s fiberoptic bronchoscopy has good nursing effect, and can effectively reduce the clinical signs, symptom relief time, hospital stay, and the incidence of postoperative complications.

Health risk and prediction analysis of adolescents caused by air pollution in Chengdu city

Objective To explore the health risk of adolescents caused by air pollutants in Chengdu, and to predict the trend of health risk in the next five years. Methods Data of air pollutants in Chengdu from January to December 2020 were used, the health risk assessment model was used to estimate the health risk of adolescents in Chengdu caused by different air pollutants, and the grey model was used to predict the future health risk. Results In 2020, the health risk of adolescents caused by air pollutants in Chengdu was in the descending order of PM2.5>PM10>O3>CO>NO2>SO2, the range was 0.73×10-9-2.76×10-6; prediction in 2025, the health risks caused by PM2.5, PM10, SO2, NO2, CO, and O3 in Chengdu will decrease by 34.4%, 40.1%, 60.7%, 78.0%, 61.8%, and 11.6%, respectively, compared with 2021. NO2 had the largest decrease, while PM2.5, PM10, and O3 had smaller decreases. Conclusion The health risks caused by PM2.5, PM10, and O3 in the atmosphere in Chengdu in 2020 are serious; prevention and control of particulate matter and O3 pollution is the focus of air pollution control in Chengdu in the next five years, and further attention should be paid to the harm caused by air pollutants to teenagers.

Construction of multi-party governance and recycling mode of expired drugs for rural residents

For a long time, expired drugs have seriously harmed the health of rural residents, it is urgent to explore the drug recycling management model. Firstly, according to the theoretical logic of “mining comparative of characteristic governance mode”, this paper constructs a multi-party governance mode centering on the government, enterprise health units, universities and scientific research institutions to overcome the defects of relying on market or government single-center governance mode. Secondly, based on the field research data Chongqin, Sichuan and Shanxi, three expired drug recovery modes and explored, namely, door-to-door recovery mode of expired drugs by rural residents, medicine delivery mode coordinated by government, enterprises and institutions, and volunteer service mood led by village cardes. Finally, combined with practice, the paper puts forward three policy suggestions to strengthen financial guarantee, strengthen policy incengtive and increase information disclosure, this paper to healp the national “healthy china stragtegy”.
