4 Study of the effect of microcapsule fluidized culture on C3A cell function and its molecular mechanism

Objective To investigate the effect of microcapsule fluidized culture on C3A cell function and its molecular mechanism. Methods Base on the different culture modes of C3A cells, they were divided into three groups: two-dimensional culture group, microcapsule static culture group and microcapsule fluidized culture group. The effect of microencapsulation fluidized culture on the activity of C3A cells, albumin, urea synthesis capacity and metabolism ability was evaluated. The expression level of genes related to hepatocyte function was detected. The expression levels of key molecules in metabolism-related signaling pathways of hepatocytes were detected. Results The albumin, urea synthesis capacity and CYP1A2, CYP3A4 metabolic activity of C3A cells in the microcapsule static culture group and microencapsulation fluidized culture group were higher than those in the two-dimensional culture group, and the differences were statistically significant (P < 0.05). Compared with the microcapsule static culture group, the albumin, urea synthesis capacity and CYP1A2 metabolic activity of C3A cells in the microcapsule fluidized culture group were increased, and the differences were statistically significant (P < 0.05). The expression levels of CYP1A2, CYP2B62, CYP2E1, CYP3A4 and UGT2B4 genes of C3A cells in the microcapsule static culture group and microencapsulation fluidized culture group were higher than those in the two-dimensional culture group, and the differences were statistically significant (P < 0.05). Compared with the microcapsule static culture group, the expression levels of p-PKC, p-PKA and calmodulin (CaM) proteins related to metabolism of C3A cells in the microcapsule fluidized culture group were decreased, and the differences were statistically significant (P < 0.05). Conclusion Microcapsule fluidized culture can significantly improve the synthesis and metabolism fuction of C3A cells, and its mechanism of action may be related to the down-regulation of the phosphorylation level of related signaling pathway proteins and level of CaM.

9 Study on proteomics of hepatocellular carcinoma cell based on iTRAQ technology

Objective To screen the differentially expressed proteins between HepG2.2.15 cells with stable hepatitis B virus (HBV) expression and its parent HepG2 cells, the isotope tagging for relative and absolute protein quantitation (iTRAQ) technique was used. Methods The differential proteins in HepG2.2.15 cells and HepG2 cells were analyzed by iTRAQ technique, and the functional clustering and signaling pathway analysis of the differential proteins were performed by bioinformatics. Differential proteins were screened based on the cell difference factor (FC)≥ 1.2 and P < 0.05 between the two groups. Kyoto encyclopedia of genes and genomes ( KEGG) and other analysis software were used for bioinformatics analysis of differential proteins. Results A total of 856 differential proteins were screened out in this study, among which 497 differentia proteins were up-regulated and 359 differential proteins were down-regulated. GO analysis showed that differential proteins were involved in biological processes including immune process and signal transduction and transport process. Most of the different protein components were distributed in cytoplasm and extracellular. The molecular functions of differential proteins mainly focus on receptor binding and metal ion binding. KEGG analysis revealed that differential proteins were mainly involved in the regulation of many cell biological functions, including drug metabolism, chemical carcinogenesis, cytochrome P450 metabolism of foreign bodies, and mitogen-activated protein kinase signaling pathways. Conclusion The differential proteins screening based on iTRAQ technology have not been previously associated with HBV replication, and may provide clues and an experimental basis for further exploration of HBV replication.

13 Effects of HIF1α deficiency in the neurons of mediobasal hypothalamus on body weight and glucose metabolism in mice

Objective To investigate the effects of hypoxia inducible factor 1α (HIF1α) deficiency in the neurons of mediobasal hypothalamus (MBH) on body weight and blood glucose metabolism in mice. Methods Thirty HIF1αflox/flox male mice were sdected, two mice with similar body weight were randomly divided into experimental group and control group, each group of 15 mice only. The average body weight difference between the two groups was no more than 2 g. Mice in the experimental group were injected with cre adeno-associated virus AAV-hSyn-cre-GFP to selectively knock down HIF1α in MBH neurons; mice in the control group were injected with AAV-hSyn-GFP. The injection site in both groups was the neurons of MBH (bregma-1.5, midline±0.5 [both sides], dorsoventral 5.8 [skull surface]). Each side was injected with 0.5 μL of virus at a concentration of 1.45E+13 v.g./mL. The body weight and food intake of the mice were weighed every day after injection, and the serum glucose and insulin contents of the mice were detected four weeks later. Results Overall analysis of the body weight of mice showed that there were statistically significant differences between groups, time point comparison and interaction (P < 0.05). Comparison within groups: the body weight of mice in both groups on the 15th, 25th and 28th day was all higher than that on the 5th day, with statistical significance (all P < 0.05). Comparison between groups: the body weight of the experimental group on the 15th, 25th and 28th day was higher than that of the control group, with statistical significance (all P < 0.05). Overall analysis of the food intake of mice showed that there were statistically significant differences between groups, time point comparison and interaction (P < 0.05). Comparison within groups: there was no significant difference in food intake between the control group on the 15th, 25th and 28th day and the 5th day (P > 0.05), but the food intake of the experimental group on the 15th, 25th and 28th day was all higher than that on the 5th day, with statistical significance (all P < 0.05). Comparison between groups: the food intake of the experimental group on the 15th, 25th and 28th day was higher than that of the control group, with statistical significance (all P < 0.05). There was no significant difference in fasting blood glucose levels between the two groups (P > 0.05). Compared with the control group, the serum insulin content of the experimental group was increased, and the difference was statistically significant (P < 0.05). Correlation analysis showed that there was a positive correlation between serum insulin content and body weight (r = 0.660, P < 0.05). Conclusion The hypothalamic neuron HIF1α plays an important role in the regulation of body weight balance in mice under normal oxygen. Hypothalamic HIF1α deficiency can cause weight gain, increased appetite, abnormal glucose metabolism and insulin resistance in mice.

17 Professor Zeng Ping’s academic thought and summary of clinical experience in the treatment of Legg-Clavé-Perthes disease

Legg-Clavé-Perthes disease is a kind of refractory disease, easy to leave complications. Although the current surgical treatment of Legg-Clavé-Perthes disease technology has been very mature, but for children, surgery risk is high, parents have strong desire for conservative treatment. Professor Zeng Ping believes that Legg-Clavé-Perthes disease is a kind of self-limited disease, which should conform to its pathological changes, and create good conditions for the growth and repair of the epiphysis of the femoral head and the shaping of the acetabulum, so as to reduce the occurrence of the later deformities. Through the application of the four therapeutic principles of traditional Chinese medicine orthopedics and traumatology scientific dynamic and static combination, equal emphasis on muscles and bones, internal and external treatment, and doctor-patient cooperation in the conservative treatment of Legg-Clavé-Perthes disease, good clinical effect has been achieved. This paper mainly summarizes Professor Zeng Ping’s academic thought and clinical experience in diagnosing and treating Legg-Clavé-Perthes disease. Finally, a classic medical case is attached.

21 Discussion on the molecular mechanism of “Lonicerae Japonicae Flos and Scrophulariae Radix” in the treatment of atherosclerosis based on network pharmacology

Objective To explore the active ingredients and potential molecular mechanism of “Lonicerae Japonicae Flos and Scrophulariae Radix” in the treatment of atherosclerosis (AS) based on network pharmacology. Methods The pharmacology database and analysis platform of traditional Chinese medicine system (TCMSP) was used and the related literatures at home and abroad were consulted. Combined with the previous experimental research, all the active ingredients and targets related to Lonicerae Japonicae Flos and Scrophulariae Radix were found. AS related targets in DisGeNET, OMIM, GeneCards were searched. The intersection targets of drug targets and disease targets were uploaded to the String platform, and the PPI network diagram was drawn by using Cytoscape software. David database was used to analyze the GO biological process and KEGG pathway enrichment of the core target protein, and to construct the network diagram of “Lonicerae Japonicae Flos and Scrophulariae Radix”-AS disease target-signal pathway. Results Through database analysis and literature review, a total of 37 active components, 231 potential drug targets, 4410 disease targets, 173 drug and disease cross target proteins, and “Lonicerae Japonicae Flos and Scrophulariae Radix” interferes with 29 core target proteins of AS were obtained. GO bioprocess analysis identified 49 items according to the screening requirements. KEGG enrichment results showed that there were 55 pathways related to AS, including tumor necrosis factor signaling pathway, Toll like receptor signaling pathway, mitogen activated protein kinase signaling pathway, etc. Conclusion It is necessary to explore the potential molecular mechanism of “Lonicerae Japonicae Flos and Scrophulariae Radix” compatibility against AS from a holistic perspective, and to explore the research mode of “multi-component of traditional Chinese medicine/bioinformatics/multi-target multi-channel action/experimental verification”, so as to provide a fast and accurate research strategy for elucidating the effective substances and mechanism of traditional Chinese medicine.

27 Bibliometrics and visualization analysis of the domestic research of mitochondrial biogenesis based on CiteSpace software

Objective To explore the current situation, hotspots and trends of mitochondrial biogenesis in recent 20 years in China based on bibliometrics. Methods The literatures on mitochondrial biogenesis published from January 2000 to June 2020 in China National Knowledge Internet were searched. CiteSpace was used to conduct co-occurrence analysis with authors, institutions and keywords as nodes, cluster analysis, time evolution analysis and burst analysis of keywords. Corresponding visualization maps were drawn and analyzed. Results There were 168 related literatures. State Key Laboratory of Silkworm Genome Biology in Southwest University and College of Food Science in Southwest University published the most papers. Ding Xiaowen and Ning Junli published the most papers, but they were lack of cooperation. Domestic research focused on mitochondrial biogenesis signaling pathway, the interaction between mitochondrial biogenesis and oxidative stress, mitochondrial autophagy, and mitochondrial apoptosis, as well as the epigenetic modification of proteins related to mitochondrial biogenesis. Conclusion CiteSpace is used to analyze research on mitochondrial biogenesis, which shows the research situation of mitochondrial biogenesis in the recent 20 years, forming a general understanding of research hotspots and trends, providing reference for the research direction.

32 Research progress on the relationship between iron and thyroid diseases

Thyroid gland is an important endocrine gland in human body, and its synthesis and secretion of thyroid hormones are of great significance for cell proliferation and differentiation, metabolism, growth and development of the body. With the improvement of living standard and the increase of work pressure, the prevalence of thyroid diseases is increasing year by year. As an essential trace element, iron plays an important role in the stability of physiological function of the body and is closely related to the synthesis and metabolism of thyroid hormone. Both iron deficiency and overload can affect thyroid function. This paper briefly reviewed the research progress of iron and thyroid diseases, in order to expand the ideas for the clinical treatment of thyroid diseases.

Surgically-induced brain injury (SBI) refers to the healthy brain surrounding the surgical lesion suffers inevitable brain injury due to various factors such as the cutting and retraction of the brain tissue by surgical instruments, and intraoperative bleeding during the implementation of neurosurgery. This special brain injury is one of the important factors affecting the prognosis of surgery which will cause secondary lesions such as brain edema, cerebral hemorrhage and neuronal death. In recent years, basic research on SBI has made some progress. In this paper, the construction methods of SBI animal models, related molecular mechanisms and treatment methods are review, in order to provide the basis for exploring the pathogenesis of SBI and provide reference for the application and development of clinical treatment methods.

45 Research progress on diagnosis and treatment of early gastric cancer under endoscopy

With the continuous development of endoscopy technology, the diagnosis technology of early gastric cancer is also maturing. In addition to white light endoscopy, the current dyeing endoscopy, magnifying endoscopy, ultrasound endoscopy, narrowband imaging technology, confocal laser microendoscopy, etc. It has been widely used, and the combined application of different inspection methods has greatly improved the detection rate of early gastric cancer. The treatment of early gastric cancer is also constantly developing. At present, the clinical application of endoscopic mucosal dissection is the most widely used. Some new endoscopic treatment methods are also constantly being explored and improved, resulting in endoscopic full-thickness gastric wall resection. New surgical procedures, such as laparoscopic combined endoscopic surgery, provide new ideas for the treatment of early gastric cancer.

49 Progress in the feasibility of trabecular bone score in predicting fracture risk in endocrine metabolic diseases

The trabecular bone score (TBS) is a gray-scale structural index generated by the lumbar dual-energy X-ray bone densitometer (DXA) image data, and is closely related to bone microstructure independent of bone density (BMD), and can predict clinical risk of upper osteoporotic fractures. At present, BMD is the most commonly used method for clinical assessment of fracture risk, but it has some limitations in evaluating bone microstructure. TBS can be used to assess bone microstructure and texture, so in people with endocrine metabolic diseases and bone structural changes, TBS can be used as a complementary tool for non-invasive assessment of bone quality and fracture risk. This paper reviews the latest research results of TBS on fracture risk assessment in this population.

53 Research on the current status and influencing factors of nurses’ risk perception in Xi’an

Objective To investigate the current situation of risk perception of nurses in Xi’an and to analyze the influence factors. Methods From September to November 2015, convenience sampling was adopted, and a total of 608 first-line clinical nurses in Xi’an were selected as the research objects. The general information questionnaire and nurses’ risk perception questionnaire (RPQN) were used to conduct a cross-sectional survey of risk perception. Results The risk perception score of nursing staff in Xi’an was (87.50±16.83) points, and the score rate was 62.50%, which was at a medium level. The top three dimensions of the score rate were physical function, occupational exposure, and time risk. The results of single factor analysis showed that there were statistically significant differences in the total risk perception scores of nursing staff of different ages, hospital levels, departments, years of service, education, and professional titles (P < 0.05). The results of multiple linear regression analysis showed that professional titles and hospital grades were the influencing factors of nurses’ risk perception (P < 0.05), explaining a total of 3.4% of the variance. Conclusion The risk perception of nursing staff in Xi’an is at a medium level. Nursing managers should optimize the nursing workflow and staffing, focusing on the risk perception level of nurses and nurses in tertiary hospitals.

57 Investigation on the status quo of rehabilitation service and the demand of rehabilitation talents of pension institutions in Shaanxi Province

Objective To explore the status quo of elderly rehabilitation service and the demand of elderly rehabilitation service talents of pension institutions in Shaanxi Province. Methods From August 2019 to July 2020, a questionnaire survey was conducted on the status of rehabilitation services in 22 elderly care institutions in Shaanxi Province. The development of rehabilitation services, the needs and training of rehabilitation talents, the main existing problems and the rehabilitation of the elderly, and the current status of the server were included in main contents. Results Among the 22 elderly care institutions in Shaanxi Province, a total of 16 provide rehabilitation services (72.72%), but still focus on simple traditional rehabilitation (54.55%). The needs of rehabilitation caregivers were the most urgent in various elderly care institutions, and college-level education requirements dominate. The willingness of elderly care institutions to cooperate with colleges and universities to jointly train elderly rehabilitation talents was not strong, only three pension agency, accounting for 13.64%. Insufficient funds and lack of rehabilitation professional elderly care service providers were the main problems in most elderly care institutions. Satisfaction of senior care positions was average, and most people think that the matching rate of knowledge acquired during school and actual senior care positions was not high. Conclusion The elderly rehabilitation services in Shaanxi Province mainly focus on traditional rehabilitation, but at the same time, it faces problems such as lack of willingness to cooperate with institutions to train rehabilitation talents, insufficient funds, shortage of rehabilitation talents, low matching rate of knowledge and demand, and low satisfaction of service providers.

61 Influencing factors of nursing needs in patients with ultrasonic gastroscopy during conscious sedation

Objective To observe the nursing needs of patients undergoing ultrasound gastroscopy during conscious sedation, and to discuss related influencing factors. Methods A total of 87 patients undergoing ultrasound gastroscopy from the Endoscopy Center of Shantou Central Hospital, Guangdong Province from February 2018 to October 2019 were enrolled, and a baseline data survey form was designed to inquire and record the general information of the patients in detail before the implementation of the gastroscopy. A self-made sober and sedative nursing questionnaire was used to assess the nursing needs of patients, and patients with a total score of more than eight scores were included in the high-need care group, and those with a score of less than eight scores were included in the low-need care group. All possible factors were included, while single factor and multivariate logistic regression analysis were used to find out the factors affecting the high demand for nursing needs in patients with ultrasonic gastroscopy during conscious sedation. Results During the ultrasound gastroscopy of 87 patients, a total of 62 patients had low demand for conscious and sedative care, accounting for 71.26%. There were statistically significant differences in the understanding of gastroscopy diagnosis and treatment, education level, operating years of doctors, using of Lidocaine hydrochloride glue, and severe anxiety before the examination among patients in different nursing needs groups (P < 0.05). The results of a single regression analysis showed that patients’ ignorance of gastroscopy diagnosis and treatment, low level of education, short operating years of doctors, using of Lidocaine hydrochloride glue, and severe anxiety before the examination were all influencing factors of the high demand for nursing needs in patients with ultrasonic gastroscopy during conscious sedation (OR > 1, P < 0.05). The results of multivariate logistic regression showed that patients’ ignorance of gastroscopy diagnosis and treatment, low level of education, short operating time of doctors, using of Lidocaine hydrochloride glue, and severe anxiety before the examination were all factors affecting the high demand for nursing needs in patients with ultrasonic gastroscopy during conscious sedation(OR > 1, P < 0.05). Conclusion Some patients undergoing ultrasound gastroscopy have a high demand for care during the conscious and sedation period, which may be related to factors such as patients’ ignorance of gastroscopy diagnosis and treatment, low education, short operating years of doctors, using of Lidocaine hydrochloride glue, and severe anxiety before the examination. Clinically, targeted interventions can be carried out accordingly.

66 Influencing factors of cognition and behavior of postpartum breastfeeding in elderly full-term cesarean section women

Objective To analyze the influencing factors of cognitive and behavior of postpartum breastfeeding in elderly full-term cesarean section women. Methods A prospective selection of 523 elderly full-term cesarean women who completed their delivery at Shanghai First Maternity and Infant Hospital from June 2019 to January 2020. The breastfeeding cognitive behavior scale was used to assess the postpartum breastfeeding cognition at 12 h postpartum. The cognitive behaviors were divided into good feeding cognitive behavior group (287 cases) and poor feeding cognitive behavior group (236 cases). The baseline data, through single factor and multivariate analysis of two group were counted and compared, the factors affecting the poor cognitive behavior of postpartum breastfeeding were found. Results There were statistically significant differences in breastfeeding knowledge, family support function, living environment, family monthly income, and visual analog scale (VAS) at 12 h postpartum between two groups (P < 0.05). There were no statistically significant differences between the two groups in age, pregnancy times, education level, medical insurance, pregnancy comorbidities, postpartum complications, and postpartum feeding time (P > 0.05). After single logistic regression analysis, poor understanding of breastfeeding knowledge, poor family support function, rural living environment, family monthly income <3000 yuan, and VAS at 12 h postpartum were all influencing factors of behavior in cognitions of postpartum breastfeeding for elderly full-term cesarean women (OR > 1, P < 0.05). Conclusion The poor cognitive behavior of postpartum breast feeding in elderly full-term cesarean section women may be affected by understanding of breast feeding knowledge, family support function, living environment and other factors in women, the early postpartum evaluation of the elderly full-term cesarean section women should be focused on clinically, and implementing reasonable intervention according to the risk factors.

71 Correlation between homocysteine and blood glucose fluctuation in patients with type 2 diabetes with vascular complications

Objective To explore the relationship between homocysteine (Hcy) and blood glucose fluctuations in patients with type 2 diabetes with vascular complications. Methods A retrospective analysis of the clinical data of 497 patients with type 2 diabetes who were admitted to the Affiliated Hospital of Xuzhou Medical University from July 2017 to April 2019. According to the presence or absence of vascular complications, the selected subjects were divided into group A (without vascular complications, 162 cases), and group B (with vascular complications, 335 cases). In addition, the patients were divided into four groups according to the Hcy quartile. Among them, Hcy≤11.0 μmol/L was the Q1 group (103 cases), 11.0 μmol/L<Hcy≤14.2 μmol/L was the Q2 group (157 cases), 14.2 μmol/L <Hcy≤19.0 μmol/L was the Q3 group (121 cases), and Hcy>19.0 μmol/L was the Q4 group (116 cases). The relative indicators of blood glucose fluctuations among the groups were compared. Results The age, course of disease, systolic blood pressure (SBP), Hcy, maximum blood glucose fluctuation range (LAGE), postprandial blood glucose fluctuation range (PPGE), and average blood glucose standard deviation (SDBG) in group B were higher than those in group A, and the differences were statistically significant (P < 0.05). There were statistically significant differences in age, course of disease, SBP, SDBG, MBG, LAGE, HOMA-β, and HbA1c in the Q1-Q4 groups (P < 0.05). Among them, age, course of disease, HbA1c, SDBG, MBG, LAGE of Q4 group were higher than those of the other three groups, Q3 group was higher than Q1, Q2 group, Q2 group was higher than Q1 group, and the differences were statistically significant (P < 0.05). SBP of Q4 group was higher than Q1 and Q3 group, lower than Q2 group; Q3 group was higher than Q1 group, lower than Q2 group; Q2 group was higher than Q1 group, and the differences were statistically significant (P < 0.05). HOMA-β in Q4 group were higher than Q1 group, lower than Q2 and Q3 group; Q3 group was higher than Q1 group, lower than Q2 group, Q2 group was higher than Q1 group, and the differences were statistically significant (P < 0.05). Correlation studies showed that Hcy was negatively correlated with body mass index, diastolic blood pressure, and total cholesterol (P < 0.05); it was positively correlated with age, SBP, HbA1c, LAGE, MBG, fasting blood glucose, PPGE, SDBG, and the course of diabetes (P < 0.05). Conclusion Patients with type 2 diabetes have large fluctuations in blood glucose and increased Hcy, suggesting that it may increase the risk of diabetes complicated with vascular complications.

75 Clinical observation of Dapagliflozin combined with Berberine in the treatment of fragile type 2 diabetess

Objective To observe the clinical effect of Dapagliflozin combined with Berberine in the treatment of fragile type 2 diabetes. Methods A selection of 60 patients with fragile type 2 diabetes who were admitted to Huai’an Cancer Hospital in Jiangsu Province from April 2018 to March 2020 were randomly divided into observation group (30 cases) and control group (30 cases) by coin flipping. The control group was given intensive insulin therapy alone, and the observation group was treated with Dapagliflozin + Berberine. The lipid metabolism and body mass index (triacylglycerol [TG], total cholesterol [TC], low-density lipoprotein cholesterol [LDL-C], body weight [BW], body mass indexes [BMI]) before and after treatment between the two groups were compared, and treatments changes in blood glucose and insulin control indicators (glycated hemoglobin [HbA1c], fasting plasma glucose [FPG], 2 h postprandial blood glucose [2hPG], fasting insulin [FINS], C peptide [CP]), blood glucose fluctuations (postprandial blood glucose fluctuation amplitude [PPGE], mean absolute difference of blood sugar [MODD], standard deviation of blood sugar level [SDBG]), blood pressure and daily use of insulin [systolic blood pressure (SBP), diastolic blood pressure (DBP), daily use of insulin] were compared before and after treatment. The occurrence of adverse reactions between the two groups was compared. Results After treatment, the levels of TG, TC, LDL-C, BW, and BMI in the two groups were lower than before treatment, while the observation group were lower than the control group, and the differences were statistically significant (P < 0.05). After treatment, the levels of HbA1c, FPG, and 2hPG in the two groups were lower than before treatment, while FINS and CP were higher than before treatment, and the levels of HbA1c, FPG, and 2hPG in the observation group were lower than those in the control group, while FINS and CP were higher than those in the control group, and the differences were statistically significant (P < 0.05). After treatment, PPGE, MODD, and SDBG in the two groups were lower than before treatment, while the observation group were lower than the control group, and the differences were statistically significant (P < 0.05). After treatment, the daily consumption levels of SAP, DAP, and daily use of insulin in the two groups were lower than before treatment, while the observation group were lower than the control group, and the differences were statistically significant (P < 0.05). There was no statistically significant difference in the total incidence of adverse reactions between the two groups (P > 0.05). Conclusion Dapagliflozin combined with Berberine has a significant effect in the treatment of patients with fragile type 2 diabetes, which can improve blood sugar levels and has high drug safety.

79 Multivariate analysis of the formation of incarcerated ureteral stones and observation of stone removal rate by minimally invasive percutaneous nephrolithotomy

Objective To analyze the influencing factors of the formation of incarcerated ureteral stones and observe the stone removal rate of minimally invasive percutaneous nephrolithotomy (MPCNL). Methods Clinical data of 126 patients with ureteral stones admitted to the Department of Urology, the 904th Hospital of the PLA Joint Logistic Support Force from February 2017 to February 2020 were analyzed. All patients were performed MPCNL treatment, according to the incarceration of ureter stones, they were divided into non-incarcerated group (74 cases) and incarcerated group (52 cases). The factors affecting the formation of incarcerated ureteral stones were analyzed, and the stone removal rate of two groups were observed. Results Univariate analysis showed that there were statistically significant differences between two groups in the history of ipsilateral stones, combined hypertension, combined hydronephrosis, stone volume, stone transverse diameter, stone maximum cross-sectional area, stone CT value and maximum thickness of ureteral wall (UWTmax) (all P < 0.05). Multivariate analysis showed that UWTmax was an independent risk factor for the formation of incarcerated ureteral stones (OR = 3.080, 95%CI: 1.056-8.981, P < 0.05). The optimal cut-off value of UWTmax was 3.41 mm, the area under the curve was 0.871 (95%CI: 0.786-0.955), the specificity was 87.65%, and the sensitivity was 82.63%. The MPCNL stone removal rate of non-incarcerated group was higher than that of incarcerated group, and the difference was statistically significant (P < 0.05). Conclusion UWTmax is an independent risk factor for the formation of incarcerated ureteral stones. UWTmax≥3.41 mm increases the risk of incarcerated ureteral stones, and the stone removal rate after treatment with MPCNL for incarcerated ureteral stones is slightly lower.

84 Clinical effect of Tansoloxin and Levofloxacin in the treatment of chronic prostatitis

Objective To analyze the clinical effect of Tansoloxin and Levofloxacin in the treatment of chronic prostatitis. Methods From April 2017 to May 2020, 86 patients with chronic prostatitis were divided into control group and observation group according to the treatment plan, with 43 cases in each group. The control group was treated with Levofloxacin, the observation group was treated with Tansororosin and Levofloxacin. The clinical effect of two groups after treatment was analyzed. Results After three weeks of treatment, the total effective rate in the observation group was higher than that in the control group (P < 0.05). There were no significant differences in interleukin (IL)-2, IL-6, macrophage inflammatory protein-2 (MIP-2), macrophage inflammatory protein-1α (MIP-1), leukocyte and lecithin corpuscles between the two groups before treatment (P > 0.05). After three weeks of treatment the lecithin bodies in prostate fluid in the two groups increased, IL-2, IL-6, MIP-2, MIP-1α and white blood cells decreased compared with those before treatment, and the lecithin bodies in prostate fluid in the observation group were higher than those in the control group, and the inflammatory factors and white blood cells were lower than those in the control group (P < 0.05). There was no significant difference in national institutes of health chronic prostatitis index (NIH-CPSI) score between the two groups before treatment (P > 0.05). The pain, voiding symptoms, quality of life and total score of NIH-CPSI in the two groups decreased compared with before treatment, and those in the observation group were lower than the control group (P < 0.05). Conclusion Tansoloxin combined with Levofloxacin in the treatment of chronic prostatitis can reduce inflammation, improve symptoms and improve the curative effect.

88 Clinical Effect of external lithotripsy combined with traditional Chinese medicine lithotripsy in the treatment of urinary calculi

To investigate the clinical efficiency of external lithotripsy combined with traditional Chinese medicine lithotripsy in the treatment of urinary calculi. Methods A total of 646 patients who received extracorporeal shock wave lithotripsy lithotripsy in Fushun Hospital of Traditional Chinese Medicine of Liaoning Province from August 2016 to August 2020 were selected. All patients were divided into observation group and control group by random number table method, with 323 cases in each group. The control group was treated with Shenzhen new element SYS.SUI-6X extracorporeal shock wave lithotripsy, and the patients were told to take more activities and drink more water after surgery. On the basis of control group, observation group was given Chinese medicine No.4 Paishi Granule to assist in removing stones for two courses. Total effective rate, stone clearance rate, stone clearance time, analgesic time, recurrence rate after 12 months and incidence of adverse reactions were compared between the two groups after treatment. Results The total effective rate of the observation group was significantly higher than that of the control group, and the difference was highly statistically significant (P < 0.01). The stone clearance rate of the first course of treatment in the observation group was higher than that in the control group, and the difference was highly statistically significant (P < 0.01). There was no significant difference in stone clearance rate between the two groups in the second course of treatment (P > 0.05). The stone clearance time and analgesic time in the observation group were significantly shorter than those in the control group, and the difference was highly statistically significant (P < 0.01). There was no significant difference in the recurrence rate between the two groups after 12 months (P > 0.05). The incidence rate of pain and fever in the observation group was significantly lower than that in the control group, the difference was highly statistically significant (P < 0.01). Conclusion External lithotripsy combined with traditional Chinese medicine lithotripsy can make it more targeted and effective to remove urinary calculus, and reduce the incidence of disease. The combination therapy is safe and worthy of clinical promotion.

92 Clinical study of Niaoduqing Granules combined with Hydrochlorothiazide in the treatment of chronic renal failure

Objective To investigate the clinical effect of Niaoduqing Granules combined with Hydrochlorothiazide in the treatment of chronic renal failure (CRF). Methods A total of 126 patients with CRF in Fuyang Second People’s Hospital from January 2018 to January 2020 were selected, they were divided into control group (63 cases) and observation group (63 cases). The control group was treated with Hydrochlorothiazide, and the observation group was treated with Nuiduqing Granules combined with Hydrochlorothiazide, the course of treatment was 8 weeks. The effect and symptom improvement time after treatment were recorded. Inflammatory factors, renal function indexes and adverse reactions during treatment were compared between the two groups before and after treatment. Results The total effective rate of observation group was higher than that of control group (P < 0.05). The improvement time of fatigue, edema, anorexia and nausea in the observation group were shorter than those in the control group (P < 0.05). After 8 weeks of treatment, the levels of interleukin-1β, creatinine, tumor necrosis factor-α, urea nitrogen and interleukin-6 in both groups were decreased than before treatment, and the observation group was lower than the control group (P < 0.05). Compared with before treatment, after 8 weeks of treatment, the clearance rate of endogenous creatinine in both groups was increased, and the observation group was higher than the control group (P < 0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Niaoduqing Granules combined with Hydrochlorothiazide in the treatment of CRF patients can effectively promote the improvement of symptoms, reduce the micro inflammatory state, delay the process of renal failure, which is safe and reliable, and the curative effect is worthy of recognition.

96 Effect of self-disclosure training on self-perceived burden and quality of life in patients with end stage renal disease maintenance hemodialysis

Objective To explore the effect of self-disclosure training in end stage renal disease (ESRD) maintenance hemodialysis patients. Methods A total of 120 ESRD maintenance hemodialysis patients admitted to the First Affiliated Hospital of Xinjiang Medical University from January 2018 to June 2019 were selected as subjects. They were divided into 62 cases in the treatment group and 58 cases in the control group by random number table method. The control group was given maintenance hemodialysis routine nursing intervention, and the treatment group was combined with self-disclosure training on the basis of the control group. Intervention for 12 weeks, the self-perceived burden and quality of life score between the two groups were compared. Results After 12 weeks of intervention, the self-perceived burden scores of patients of the two groups were significantly lower than those before the intervention. The total scores of emotional burden, physical burden, and self-feeling burden of the treatment group were lower than those of the control group, and the differences were all statistically significant (all P < 0.05). After 12 weeks of intervention, the scores of kidney disease targeted areas, general health-related quality of life and kidney disease quality of life instrument of the two groups were higher than those before the intervention and the treatment group were higher than those of the control group, and the differences were all statistically significant (all P < 0.05). Conclusion Self-disclosure training can help relieve the burden of self-feeling in ESRD maintenance hemodialysis patients and improve the quality of life.

100 Clinical effect of Buqi Yishen Huoxue method combined with interventional therapy on early and middle stage femoral head necrosis

Objective To explore the clinical effect of Buqi Yishen Huoxue method combined with interventional therapy on early and middle stage femoral head necrosis. Methods A total of 120 patients with early and middle stage femoral head necrosis admitted to Liaocheng Traditional Chinese Medicine Hospital, Shandong Province, from June 2018 to June 2020 were selected. They were divided into control group and observation group by random number table method, with 60 cases in each group. The control group was treated with selective vascular drug interventional therapy, and the observation group was treated with Buqi Yishen Huoxue method on the basis of control group. Both groups were treated continuously for one month. Clinical efficacy and the incidence of adverse reactions were compared between two groups. Traditional Chinese medicine (TCM) syndrome scores, bone metabolism indexes before and after treatment, and hip joint function after three months follow-up were compared between the two groups. Results The total effective rate and the excellent and good rate of hip joint function in the observation group were higher than those in the control group, with statistical significance (all P < 0.05). After treatment, TCM syndrome score, Ⅰ type collagen cross-linked carboxy-terminal peptide (CTX-Ⅰ) and tartrate-resistant acid phosphatase (TRAP) in two groups were lower than those before treatment, serum osteocalcin (BGP) and Ⅰ type collagen amino-terminal elongation peptide (PⅠNP) were higher than those before treatment, and TCM syndrome score, CTX-Ⅰ and TRAP in the observation group were lower than those in the control group, BGP and PⅠNP were higher than those in the control group, and the differences were statistically significant (all P < 0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P > 0.05). Conclusion Buqi Yishen Huoxue method combined with interventional therapy on early and middle stage femoral head necrosis can improve the TCM syndrome, promote the improvement of bone metabolism indexes, and help alleviate hip function without increasing adverse reactions, with high safety.

104 Effect of traditional Chinese medicine bath of Honghua Huayu Decoction on shoulder joint function recovery and quality of life after rotator cuff injury surgery

Objective To investigate the effect of traditional Chinese medicine bath of Honghua Huayu Decoction on shoulder joint function recovery and quality of life after rotator cuff injury surgery. Methods A total of 124 patients with rotator cuff injury who underwent arthroscopic rotator cuff repair in Cangzhou Hospital of Integrated Traditional Chinese Medicine and Western Medicine of Hebei Province from March 2018 to July 2020 were selected. According to the envelope drawing method, the patients were divided into control group and study group, with 62 cases in each group. The control group was given routine rehabilitation treatment, and the study group was combined with Honghua Huayu Decoction for traditional Chinese medicine bath on the basis of the control group. Both groups were treated continuously for eight weeks. Clinical efficacy was compared beeween two groups. University of California shoulder system score (UCLA), functional independence scale (FIM), visual analog scale (VAS), inflammatory cytokines and quality of life scores were compared between the two groups before and after treatment. Results The total effective rate of the study group was higher than that of the control group, and the difference was statistically significant (P < 0.05). After treatment, the UCLA and FIM scores of the two groups were higher than those before treatment, and the VAS was lower than that before treatment, and the UCLA and FIM scores of the study group were higher than those in the control group, and the VAS was lower than that in the control group, the differences were statistically significant (all P < 0.05). After treatment, the levels of hypersensitive C-reactive protein, interleukin-8 and tumor necrosis factor-α in both groups were lower than those before treatment, and study group were lower than the control group, with statistical significance (all P < 0.05). After treatment, scores of social function, physical pain, physiological enginery, physiological function, emotional function, mental health, vitality and health status in both groups were higher than those before treatment, and the study group was higher than the control group, the differences were statistically significant (all P < 0.05). Conclusion After surgery, the patients with rotator cuff injury are given Honghua Huayu Decoction for traditional Chinese medicine bath treatment, which can effectively promote the recovery of shoulder joint function, improve the quality of life of patients, and reduce inflammation, with definite curative effect.

109 Comparative study with two ways of assisting proximal femoral nail antirotation in the treatment of unstable femoral tuberosity fractures in the elderly

Objective To investigate the clinical effect of reconstruction of femoral moment combined with proximal femoral nail antirotation (PFNA) and bone hook assisted PFNA in the treatment of senile unstable femoral tuberosity fracture. Methods Fifty-two elderly patients with unstable trochanteric fracture who were hospitalized in Nantong Tongzhou People’s Hospital, Jiangsu Province from January 2015 to January 2019 were selected, the subjects were divided into observation group and control group by digital coding according to admission sequence, with 26 cases in each group, the observation group were treated with reconstructed femoral moment combined with PFNA treatment, the control group were treated with bone hook to assist PFNA treatment. The operation time, intraoperative blood loss, Harris score, and postoperative complications of the two groups were compared. Both groups were followed up for 12 months. Results The operation time and bleeding volume of the control group were higher than that of the control group, the differences were statistically significant (P < 0.05); start time of postoperative exercise was shorter than the observation group, the difference was statistically significant (P < 0.05); the postoperative Harris score of the observation group was higher than that of the control group, the difference was statistically significant (P < 0.05); there was no statistically significant difference in the incidence of complications between the two groups (P > 0.05). Conclusion Reconstruction of femoral moment combined with PFNA and bone hook-assisted PFNA can obtain good clinical results, avoid single PFNA defects, and achieve the role of repair, reconstruction and stability of lateral wall integrity. Reconstruction of femoral moment combined with PFNA can obtain better stability and better functional recovery; the operation time of bone hook assisted PFNA is short, the amount of bleeding is less, the operation is difficult and the trauma is less. Patients with better health and strong willingness to obtain normal mobility can give priority to the reconstruction of femoral moment combined with PFNA surgery.

113 Analysis of colonization of Group B Streptococcus, antimicrobial susceptibility and pregnancy outcome in the third trimester of pregnancy

Objective To analyze the colonization and antimicrobial susceptibility of Group B Streptococcus (GBS) in the third trimester of pregnancy and its pregnancy outcome. Methods The medical records of 500 pregnant women who voluntarily underwent vaginal combined rectal GBS screening in the Second Affiliated Hospital of Anhui Medical University from January to September 2020 were retrospectively analyzed. The colonization of GBS and the antimicrobial susceptibility of GBS positive samples were analyzed. According to the results of GBS screening, they were divided into GBS negative group and GBS positive group, and the pregnancy outcome of two groups was compared. Results A total of 56 pregnant women with GBS positive were detected among 500 pregnant women, and the colonization rate was 11.20%; among which 41 were positive vaginal specimens, the colonization rate was 8.20%; there were 32 positive rectal specimens, the colonization rate was 6.40%. GBS had high resistance to Clindamycin, Erythromycin and Tetracycline, and high sensitivity to Cefuroxime, Linezolid, Vancomycin, Ampicillin and Penicillin. The proportion of prenatal fever, abnormal pregnancy history of GBS positive group were higher than those of GBS negative group, and the differences were statistically significant (all P < 0.05). The proportions of premature rupture of membranes, puerperium infection, postpartum hemorrhage, neonatal septicemia and neonatal pneumonia in GBS positive group were higher than those in GBS negative group, and the differences were statistically significant (all P < 0.05). Conclusion The overall colonization rate of GBS is low in the third trimester of pregnancy, and the detection rate of GBS can be increased by comprehensive rectal examination. GBS positive patients have higher adverse pregnancy outcomes such as premature rupture of membranes, puerperal infection, Prenatal fever postpartum hemorrhage, neonatal septicemia, and neonatal pneumonia. GBS has high sensitivity to Cefuroxime, Linezolid, Vancomycin, Ampicillin and Penicillin.

117 Correlation between MTHFR gene C677T and A1298C polymorphisms and plasma homocysteine in recurrent abortion

Objective To investigate the correlation between MTHFR gene C677T and A1298C polymorphisms and plasma homocysteine in recurrent abortion. Methods A total of 100 women with recurrent abortion admitted to Huai’an First People’s Hospital, Jiangsu Province were selected as the recurrent abortion group, and 100 women with a history of one childbirth without history of adverse pregnancy were selected as the control group from December 2016 to June 2019. Restriction fragment length polymorphisms were used to detect the MTHFR gene C677T and A1298C genotypes; the occurrence risk of recurrent abortion with different genotypes was analyzed; colorimetric method was used to detect homocysteine level; the differences in homocysteine level between different genotypes were compared. Results There were statistically significant differences in the genotypes of C677T and A1298C between the two groups (all P < 0.05). Compared with C677T CC type, C677T TT type had a 3.375-fold increased risk of recurrent abortion (P = 0.008); Compared with A1298C AA type, A1298C CC had a 3.721-fold increased risk of recurrent abortion (P = 0.012). The homocysteine level in the recurrent abortion group was higher than that in the control group, and the difference was statistically significant (P < 0.05). Homocysteine level of C677T TT type was higher than that C677T CC type and C677T CT type, and the difference was statistically significant (P < 0.05); homocysteine level of A1298C CC type was higher than that of A1298C AA type and A1298C AC type, and the difference was statistically significant (P < 0.05). Conclusion The MTHFR gene C677T TT type and A1298C CC type have a higher risk of recurrent abortion, and C677T and A1298C polymorphisms can lead to elevated homocysteine level. The MTHFR gene C677T and A1298C mutations may be involved in the pathogenesis of recurrent abortion by changing homocysteine level.

The serum levels of C-reactive protein (CRP), serum amyloid A (SAA), tumor necrosis factor-α (TNF-α), malondialdehyde (MDA) and superoxide dismutase (SOD) were compared between two groups before and after treatment. The patients were followed up for 6 months to record and observe the adverse reactions and recurrence of two groups. Results The improvement time of vulva vaginal pain, mucosal congestion, abnormal leucorrhea and vulva pruritus in observation group were shorter than those in control group, and the differences were statistically significant (all P < 0.05). The total effective rate of observation group was higher than that of control group, and the difference was statistically significant (P < 0.05). Before treatment, there were no significant differences in the levels of SAA, CRP and TNF-α between two groups (P > 0.05). After treatment, the levels of SAA, CRP and TNF-α in two groups were lower than those before treatment, and observation group was lower than control group, and the differences were statistically significant (all P < 0.05). Before treatment, there were no significant differences in SOD and MDA levels between two groups (P > 0.05). After treatment, SOD level of two groups were higher than those before treatment, and observation group was higher than control group; MDA level of two groups were lower than those before treatment, and observation group was lower than control group, and the differences were statistically significant (all P < 0.05). During the treatment, no obvious adverse reactions were observed in two groups. During the follow-up, no cases were shed. Three and six months after treatment, the recurrence rate of observation group were lower than those of control group, and the differences were statistically significant (all P < 0.05). Conclusion Lactobacillus Vaginal Capsules combined with Nifuratel Tablets in the treatment of senile bacterial vaginitis can improve symptoms, reduce oxidative stress, and reduce the level of inflammatory factors, the curative effect is definite.

125 Effect of regulating menstruation and promoting pregnancy acupuncture in the treatment of patients with premature ovarian insufficiency undergoing in vitro fertilization and embryo transfer

Objective To observe the effect of regulating menstruation and promoting pregnancy acupuncture in the treatmen of patients with premature ovarian insufficiency (POI) undergoing in vitro fertilization and embryo transfer (IVF-ET). Methods From November 2018 to October 2020, Eighty POI patients who admitted to Department of Acupuncture and Moxibustion, Shenzhen Traditional Chinese Medicine Hospital and Department of Chinese Medicine, Shenzhen Maternal & Child Healthcare Hospital Affiliated to Southern Medical University and planned to undergo IVF-ET were selected as the research subjects. They were divided into the acupuncture group and the control group according to the random number table method, with 40 cases in each group. All patients were preconditioned artificially for three months, followed by an antagonist regimen for ovulation induction. The control group was not given any acupuncture treatment in the whole process, while the acupuncture group was given acupuncture therapy. The sex hormone levels, gonadotropin use between the two groups were compared before and after treatment; the number of mature follicles (≥1.8 cm), endometrial thickness, the level of estradiol (E2) and the number of eggs obtained on injection human chorionic gonadotropin (hCG) day. Results Before and after treatment, there were no significant differences in the levels of basic follicle-stimulating hormone (bFSH), basic luteinizing hormone (bLH), basic estradiol (bE2) and antimullerian hormone (AMH) between the two groups (P > 0.05). After treatment, the levels of bFSH and bLH in both groups were lower than those before treatment, and the differences were highly statistically significant (all P < 0.01). There were no significant differences in bE2 and AMH between the two groups after treatment and before treatment (P > 0.05). The amount of Urinary Follicle-Stimulating Hormone for injection, the starting amount and total use time in the acupuncture group were lower than those in the control group, and the differences were highly statistically significant (all P < 0.01); On hCG day, the number of mature follicles ≥1.8 cm, endometrial thickness, the proportion of A+B type endometrium and the number of eggs obtained were higher than those in the control group, and the differences were statistically significant (P < 0.05 or P < 0.01); the level of E2 in the acupuncture group was higher than that in the control group, and the difference was statistically significant (P < 0.05). Conclusion Regulating menstruation and promoting pregnancy acupuncture can improve the sensitivity to gonadotropin, increase the thickness of endometrium, improve the ovarian ovulation function and endometrial receptivity.

129 Comparison of the application of lateral position and lithotomy position in the second stage of labor nursing of full-term primiparas

Objective To compare the application effect of lateral position and lithotomy position in the second stage of labor nursing for full-term primiparas, to guide the nursing intervention for the second stage of labor for full-term primiparas. Methods From April 2018 to April 2020, 120 full-term primiparas in the First Affiliated Hospital of Nanjing Medical University were selected as the research objects. They were divided into observation group (60 cases) and control group (60 cases) by random number table method. Observation group was used the lateral position during the second stage of labor, and control group was used the lithotomy position during the second stage of labor. The duration of the second stage of labor, the vaginal delivery rate, the perineal pain degree (visual analogue scale [VAS] assessment) at 2, 4, 8 h after delivery, and the incidence of perineal injury and perineal lateral resection between two groups were compared. Results The second stage of labor in observation group was shorter than that in the control group, and the vaginal delivery rate was lower than that in control group, and the differences were significant statistically (all P < 0.05). VAS score at 8 h after delivery was lower than that at 2, 4 h after delivery, VAS score at 4 h after delivery was lower than that at 2 h after delivery; VAS scores of observation group at each time point were lower than those of the control group, and the differences were significant statistically (all P < 0.05). The incidence of perineal injury and perineal lateral resection in observation group were lower than those in control group, and the differences were significant statistically (all P < 0.05). Conclusion The use of lateral position in the second stage of labor for full-term primiparas are beneficial to shorten the second stage of labor, effectively reduce vaginal delivery, and reduce the degree of perineal pain, perineal injury and perineal lateral resection.

133 Comparison of the key drivers of outpatient satisfaction in tertiary traditional Chinese medicine hospitals and general hospitals in China

Objective To study the key influencing factors of outpatient satisfaction in tertiary traditional Chinese medicine hospitals and general hospitals in China and their similarities and differences, and to provide specific suggestions for improving the quality of medical services in the two types of hospitals. Methods The Likert 5 levels patient satisfaction scale was used to investigate the satisfaction of outpatients in traditional Chinese medicine hospitals and general hospitals. The importance matrix analysis method was used to identify the key drivers of outpatient satisfaction in the two types of hospitals. Results The common key advantages of the two types of hospitals included “diagnosis and treatment service”, “medical emotional care” and “hospital guidance sign”, while the common key disadvantages were “treatment time” and “spatial layout”. At the same time, the key advantages of traditional Chinese medicine hospitals included “consultation service”, the key disadvantages included “service window”, and the key disadvantages of general hospitals included “waiting time” and “environment of rest and waiting area”. Conclusion The overall satisfaction level of outpatients in tertiary hospitals in China is relatively high, and the key advantages of the two types of hospitals are basically the same. Spatial layout and treatment time are areas that need further improvement in both types of hospitals. Meanwhile, compared with general hospitals, traditional Chinese medicine hospitals should pay more attention to the service attitude of window staff, while general hospitals should pay more attention to the optimization of medical treatment process and improve the environment of rest and waiting area.

137 Study on the construction of a survey tool for continuity of care in close county medical community

Objective To construct a survey tool for continuity of care in close county medical community and to provide empirical evidence for the construction and improvement of close county medical community. Methods Based on the theoretical framework of “relationship continuity, information continuity and management continuity”, literature analysis and thematic frame analysis were used to compile questionnaire items. A questionnaire survey was conducted among 250 residents in close county medical community in Dancheng County, He’nan Province in December 2020 to evaluate the reliability and validity of the questionnaire. Results Based on the 57 literatures finally included, eight key elements of evaluation were summarized and extracted, and a survey tool containing ten items was preliminarily formed. The overall Cronbach’s α coefficient was 0.82, with high reliability. The results of factor analysis showed that the structural validity of this survey tool was good. Conclusion A survey tool for continuity of care in close county medical community with good reliability and validity is preliminarily constructed, which can provide reference for the evaluation of integrated medical and health care system in the future.

141 Preparation and validation of a national reference for nucleic acid test reagent of Streptococcus pneumoniae

Objective To prepare a national reference for nucleic acid test reagent of Streptococcus pneumoniae and validate its applicability. Methods Ten strains of Streptococcus pneumoniae and ten strains of non-Streptococcus pneumoniae were selected to prepare positive reference, negative reference, repeatable reference and minimum detection limit reference. The uniformity and stability of the reference package was evaluated. The accuracy, specificity, reproducibility and minimum detection limit of the prepared national reference of Streptococcus pneumoniae were validated by using nucleic acid detection kits of Streptococcus pneumoniae produced by four enterprises. Results The packing uniformity of the reference was good, and the coefficient of variation of the cycle threshold values were all within 5.00%. The stability of the reference was not affected by being placed for three or seven days at 2-8℃,room temperature (25℃) and 37℃ and repeated freezing-thawing for three or five times. The accuracy, specificity and reproducibility of the kits from the four enterprises were all in line with the requirements. The minimum detection limit of the kits from one enterprise was 1.0×104/mL, and the minimum detection limit of the kits from the other three enterprises was 1.0×103/mL. Conclusion A set of national reference reagents for nucleic acid test reagent of Streptococcus pneumoniae has been prepared, which can be used for the quality control and evaluation of Streptococcus pneumoniae nucleic acid detection kits.

145 Effect of Azithromycin combined with Cefotaxime in the treatment of AECOPD and its influence on pulmonary function

Objective To investigate the efficacy of Azithromycin combined with Cefotaxime in the treatment of acute exacerbation of chronic obstructive pulmonary disease (AECOPD) and its influence on pulmonary function. Methods A total of 80 patients with AECOPD admitted to the Department of Emergency Internal Medicine, Bozhou Hospital of Traditional Chinese Medicine, Anhui Province from February 2018 to February 2020 were selected as the study subjects. They were divided into observation group and control group by random number table method, with 40 cases in each group. The control group was treated with Cefotaxime, and the observation group was treated with Azithromycin on the basis of control group. Both groups were treated for one week. The curative effect and the charges of pulmonary function indexes (forced vital capacity [FVC], forced expiratory volume in one second [FEV1], percentage of FEV1 to FVC [FEV1/FVC]), C-reactive protein (CRP) and procalcitonin (PCT) before and after treatment were compared between the two groups. The occurrence of adverse reactions was recorded. Results The total effective rate of the observation group was significantly higher than that of the control group (P < 0.05). After treatment, FVC, FEV1 and FEV1/FVC in two groups were higher than those before treatment, CRP and PCT levels were lower than those before treatment, while the pulmonary function indexes in observation group were all higher than those in control group, CRP and PCT levels were lower than those in control group, the differences were statistically significant (all P < 0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P > 0.05). Conclusion Azithromycin combined with Cefotaxime is effective and safe in the treatment of AECOPD.

Based on the literature review of ancient Chinese medicine books, this paper sorts out nearly ten concepts of the relationship between teeth and bone, such as “zhong”“yu”“rong”“biao”“hou”, explaines their meanings, and clarifies the general context of the theoretical evolution. Among them, Lingshu Jing explains the relationship between the five zang organs and the five flavors by “the tooth is the end of the bone”, which is the source of its theory. At the same time, under the theory guidance of “teeth are the surplus of bones”, teeth become the indicators to observe the rise and fall of kidney essence and the growth and aging of human body. This theory is often used to explain the pathogenesis of cold limbs, atrophic debility of bones, and many dental diseases, which is mainly related to kidney qi deficiency or yin deficiency and fire excess. During Ming and Qing Dynasties, the theory of epidemic febrile diseases rose, on the basis of which the tooth examination method is derived, which become the unique method for the diagnosis of epidemic febrile diseases. Kidney tonifying herbs and bitter herbs are commonly used to tonify bones and teeth, as well as a large number of explanations of the mechanism of bone drugs, which is the embodiment of the theory to guide clinical medication. This paper mainly reviews from the above aspects of the origin and development of the theory of the main tributaries of the system.

[Abstract] Objective To investigate the effect of electric nape acupuncture combined with drugs on the cognitive dys function of patients with cerebral small vessel disease (CSVD). Methods A total of 72 patients with CSVD admitted to the outpatient department and ward of Second Affiliated Hospital of Heilongjiang University of Chinese Medicine from April 2018 to October 2020 were selected and they were divided into control group and treatment group by random number table method, with 36 patients in each group. The control group received oral medicine, acupuncture, symptomatic basic treatment. The treatment group was combined with electric nape acupuncture therapy on the basis of the control group, and both groups were treated for eight weeks. The Montreal cognitive assessment scale (MOCA) and the mini-mental state examination (MMSE) were used to assess the two groups before, two weeks and eight weeks after treatment, respectively. Results Of the 72 patients, 69 cases were actually observed and three cases were abscission. Among them, 35 cases in the control group and 34 cases in the treatment group completed observation. There were statistically significant differences in comparison between groups, time point comparison and interaction (P < 0.05). Comparison within groups: MoCA and MMSE scores of the two groups after eight weeks of treatment were significantly higher than those before and after two weeks of treatment, the differences were statistically significant (all P < 0.05). There were no significant differences in MoCA and MMSE scores between the two groups before treatment and two weeks after treatment (P > 0.05). Comparison between groups: there were no significant differences in MoCA and MMSE scores between the two groups before treatment and two weeks after treatment (P > 0.05). After eight weeks of treatment, MoCA and MMSE scores in the treatment group were significantly higher than those in the control group, and the differences were statistically significant (all P < 0.05). Conclusion Electric nape acupuncture therapy can improve the MMSE and MoCA scores of patients with cognitive dysfunction of CSVD, and improve the cognitive function, and the longer the course of treatment, the better the benefit.

Usually, “reconciliation” is the method to regulate the qi of the human body by eliminating the discharge, harmonizing gastrointestinal tract, harmonizing liver and spleen, harmonizing inner and external treatment. This paper listes eight clinical cases of applying Wendan Decoction based on syndrome differentiation: pulmonary infection, postpartum cold, coronary heart disease, chronic gastritis, lipoma, cervical dizziness, fatty liver and postoperative cancer. Through the analysis of the above cases, the clinical experience of applying Wendan Decoction is summarized, and the dialectical thinking of inheriting Feng Chonglian, a famous traditional Chinese medicine doctor in Guangdong Province, is provided for the clinical application of Wendan Decoction.

167 Study on medication rule of traditional Chinese medicine in treating precocious puberty of girls based on literature

Objective To analyze the medication rule of traditional Chinese medicine in treating precocious puberty of girls based on data mining method. Methods From the database establishment to June 2020, the published journals in China biomedical literature service system, China journal full text database, Wanfang data knowledge service platform, VIP Chinese science and technology journal database, PubMed and ScienceDirect were searched by computer. The data was input into the ancient and modern medical record cloud platform (V2.2.3) to analyze the medication rules of traditional Chinese medicine in the treatment of girls’ precocious puberty, and compared with Fuyou Mixture. Results A total of 3332 Chinese literatures, 21 Western literatures and 213 included literatures were searched, including 226 prescriptions. The main traditional Chinese medicine syndrome was yin deficiency and fire hyperactivity. The frequency of traditional Chinese medicine from high to low was Anemarrhenae Rhizoma, Rehmanniae Radix, Phellodendri Chinensis Cortex, Moutan Cortex, Prunellae Spica. The common Chinese medicines were mainly cold and bitter in nature, and mostly belong to liver and kidney channels. The core prescription was Zhibai Dihuang Pills, there were four kinds of traditional Chinese medicine with Fuyou Mixture. Conclusion The principle of traditional Chinese medicine in treating precocious puberty of girls is liver, spleen and kidney. Based on this result, we plan to add Anemarrhenae Rhizoma and Phellodendri Chinensis Cortex to improve Fuyou Mixture.

This paper systematically summarizes the discussion on palpitation in Shanghan Zabing Lun, and points out the different clinical features, etiology and pathogenesis of palpitation caused by the disease of the heart itself and the disease of each viscera involving the heart, summarizes the medication characteristics of palpitation caused by different viscera diseases and different pathological products, and puts forward the concepts of “stomach heart disease”, “gallbladder heart disease”, “liver heart disease” and “kidney heart disease” to remind clinicians that when treating palpitations, they should not only consider the heart problem, but also consider whether the heart is caused by other viscera problems. 180 Effect on the ultrasound-guided akupotomye technique in the treatment of cervical spondylosis

Objective To observe the clinical efficacy of ultrasound-guided akupotomye technique in the treatment of cervical spondylosis. Methods A total of 200 patients with cervical spondylosis admitted to Affiliated People’s Hospital of Fujian University of Traditional Chinese Medicine from October 2017 to June 2020 were selected. According to random number table method, they were divided into treatment group and control group, with 100 cases in each group. Ultrasound-guided akupotomye was used in the treatment group and traditional akupotomye was used in the control group. The treatment course of both groups was two weeks. The changes of visual analogue score (VAS) and Japanese Orthopaedic Association (JOA) scale score before and two weeks after treatment were observed. The changes of cervical curvature and surface electromyography (sEMG) of trapezius and sternocleidomastoid before and three months after treatment were observed in the two groups. Results Before treatment, there were no significant differences in VAS, JOA score, cervical curvature, trapezius and sternocleidomastoid sEMG between the two groups (P > 0.05). After two weeks of treatment, the VAS of the two groups was lower than that before treatment, and the JOA score of the two groups was higher than that before treatment, and the VAS of the treatment group was lower than that in the control group, and the JOA score of the treatment group was higher than that in the control group, with statistical significance (all P < 0.05). Three months after the treatment, the cervical curvature and the sEMG of trapezius and sternocleidomastoid in both groups were higher than those before treatment, and the treatment group were higher than the control group, with statistical significance (all P < 0.05). Conclusion Ultrasound-guided akupotomye technique can significantly improve the neck function of patients with cervical spondylosis, reduce neck pain, improve the anti-fatigue ability of their muscles, and restore the physiological curvature of cervical spondylosis, and its curative effect is better than that of traditional akupotomye therapy.

184 Experience on the application of customer relationship Management in the business development department of Xiaotangshan sanatorium in Beijing

Objective To explore the significance and value of the concept and practice of customer relationship management in customer maintenance of sanitarium. Methods The customer information of the Nursing Home Business Development Department from January 2015 to December 2019 were digged, the annual income, cooperation volume of different types of customers, cooperation volume of group customers, loss of group customers and the number of third-party operating customers were taken as evaluation indexes, the application strategy of customer relationship management in customer maintenance and sustainable development of the nursing home business development department were explored. Results Compared with 2015, the overall annual revenue brought by the implementation of customer relationship management showed a steady rise, the total revenue in 2019 increased by about 10% year-on-year, and the highest proportion of income from physical examination (about 71.95% in five years), the total revenue from physical examinations increased by about 14.71% year-on-year in 2019; the number of VIP customers among different types of cooperation customers increased by 18.35% year-on-year in 2019, and the number of special mission customers increased by 43.85% in 2019; five years of stable cooperation with group customers (annual volatility of 7.09%), overall decline in group customer loss (50% by 2019), since the addition of third-party cooperation in 2016, the number of customers in 2019 has increased by 322% compared with the same period in 2016. Conclusion Customer relationship management can effectively increase the number of nursing home customers, reduce customer loss, and maintain a good, long-term customer cooperation relationship, at the same time, the effectiveness of third-party cooperation in customer growth and maintenance is obvious, it is a kind of recommended customer relationship management mode, which lays a strong foundation for better developing potential customers and establishing customer relationship management platform of sanatorium.

188 A comparative analysis of operation efficiency of secondary public hospitals in a central province based on data envelopment analysis

Objective To measure and analyze the operational efficiency of secondary public hospitals in a central province (SX) of China, to improve the efficiency of hospital service to provide decision-making reference. Methods A total of 154 secondary general hospitals in SX Province were taken as the research objects, a total of four input indicators and three output indicators of 2018 were selected for data envelopment analysis (DEA), hospital operating efficiency were measured. Results The mean values of comprehensive technical efficiency, pure technical efficiency and scale efficiency of secondary public hospitals in SX Province were 0.857, 0.904 and 0.946, respectively. A total of 33 hospitals (21.43%) were in the DEA effective state, and 121 hospitals (78.57%) were in the non-DEA effective state. Conclusion The overall efficiency of secondary hospitals in SX Province was relatively high, and hospitals with diminishing returns of scale account for the largest proportion of secondary hospitals. It is necessary to focus on the risk of scale expansion, further improve the level of management, and strengthen tiered diagnosis and treatment.

192 Mental health status of the elderly in China and intervention suggestions

This paper reviews and summarizes the literature on mental health of the elderly in China since 2014, it is concluded that there are some differences in mental health influencing factors and manifestations of urban elderly, rural elderly, retired elderly and empty-nesters in China, the mental health problems of the elderly in rural areas are more serious than those in urban areas, chronic diseases, low educational attainment and poverty are major factors. The psychological problems of retired old people appear in the form of “retired syndrome”, the mental health status of the military retired elderly and the university retired elderly subgroup is also different. The main psychological problems of empty nesters are “empty nest syndrome” caused by economic status and family factors. This paper puts forward some suggestions on mental health intervention policies for the elderly, such as carrying out mental health education, strengthening community activities, comprehensively improving social support ability, and establishing community follow-up management and referral mechanism.
