4 Comparative analysis of Chinese and American guidelines for the diagnosis and treatment of cancer pain
LI Hui1 RAO Yuefeng2
Drug therapy is the most important and commonly used method for the treatment of cancer pain. Regular and effective drug therapy can relieve 80%-90% of the pain symptoms of cancer patients. For a long time, the Clinical Practice Guidelines for Adult Cancer Pain issued by the National comprehensive cancer network (NCCN) of the United States in January every year has been one of the important references for doctors around the world in the diagnosis and treatment of cancer pain. The former National Health and Family Planning Commission of China issued two versions of the Standards for the Diagnosis and Treatment of Cancer Pain in 2011 and 2018 respectively. The guidelines of the two countries are different in the suggestions of the aspects of pain assessment, pain grade, dose titration, maintenance medication, dose conversion, opioids reduction and the prevention and treatment of opioid adverse reactions. In this paper, from the perspective of clinical pharmacists, the classification and comparative interpretation of the diagnosis and treatment of cancer pain in China and the United States are carried out, hoping to provide pharmaceutical suggestions for clinicians in the diagnosis and treatment of cancer pain.

QIN Zhenying1 ZHONG Fengyu2 CHEN Yao2 LI Jing1 YANG Zi1 WEN Juan3 WANG Yumei4 HU Youfang1
Objective To investigate the regulatory effect of adipokines tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) on the expression of miR-506 in mature human adipocytes. Methods Real-time fluorescence quantitative polynucleotide chain reaction was used to detect the expression level of miR-506 in human precursor adipocytes and mature adipocytes. TNF-α and IL-6 were treated with 10, 30 ng/mL for 0, 4, 8 and 24 h, and the expression level of miR-506 was detected at each time point. Results The expression level of miR-506 in mature adipocytes was higher than that in precursor adipocytes, and the difference was highly statistically significant (P < 0.01). The expression of miR-506 in mature human adipocytes at 4, 8 and 24 h after treatment with 10 ng/mL TNF-α and IL-6 were not significantly differents from those at 0 h after treatment (P > 0.05). The expression level of miR-506 in mature human adipocytes after treatment with 30 ng/mL TNF-α and IL-6 at 24 h were higher than those after treatment at 0 h, and the differences were statistically significant (all P < 0.05). Conclusion miR-506 expression is regulated by adipokines, which provides theoretical basis for the study of the correlation between miR-506 and inflammatory responses and obesity-related insulin resistance.

ZHOU Xinpeng1,2 JIN Yehua1,2 ZHANG Runrun1,2 HE Dongyi1,2
Objective To explore the long non-coding RNA (lncRNA) -microRNA (miRNA) -mRNA network and key lncRNA in rheumatoid arthritis (RA) fibroblast-like synoviocytes (FLS), and to reveal the role of lncRNA in RA. Methods Data sets GSE103578, GSE83147 and GSE128813 were obtained from Gene Expression Omnibus (GEO) of the National Center for Biotechnology Information in the United States. Each data set contained three synovium-isolated FLS from healthy people in the control group and three FLS from RA patients in the RA group. R language program, idmap3 package was used for probe annotation. Limma package was used to standardize the data, remove batch effect and analyze gene differential expression. LogFC > 1.0 and P < 0.05 were used as the criteria for differential expression. Correlation analysis was conducted for the differentially expressed lncRNA and mRNA. In the lncbase, starBase and miRcode databases, the miRNAs related to lncRNA were obtained, and the miRNAs related to mRNA were searched in the miRWalk and TargetScan databases. The intersection of the two was taken, and the lncRNA-miRNA-mRNA network was constructed by Cytoscape software, and the lncRNAs in the network were taken as the key lncRNA. Results After standardization of GEO data set, elimination of batch effect and annotation, 14 760 mRNAs and 5290 lncRNAs were finally obtained. Compared with the control group, there were 31 mRNA up-regulated and 56 mRNA down-regulated in RA group, and 9 up-regulated genes and 14 down-regulated genes in lncRNA. The Pearson correlation coefficient R > 0.85 and P < 0.05 were used as the criteria for the co-expression of lncRNA-mRNA genes, and 18 co-expressed lncRNAs and 51 mRNAs were obtained. There were 26 miRNAs that had binding sites with co-expressed lncRNA and mRNA. In the constructed lncRNA-miRNA-mRNA co-expression network, there were 13 lncRNAs, including lncRNA XIST, NR2F2-AS1, LINC01018, etc., 26 miRNAs and 46 mRNAs. Conclusion Thirteen lncRNAs, such as lncRNA XIST, NR2F2-AS1, LINC01018, are differentially expressed in RA-FLS, and the expression of miRNA and mRNA is regulated through the competitive endogenous RNA mechanism to form a regulatory network of lncRNA-miRNA-mRNA, which could be used as the target lncRNA for further verification.

Results After treatment, VAS, PRI scores, PPI scores and PII scores of two groups were lower than those before the treatment, and scores of each item in the treatment group were lower than those in the control group, with statistical significant differences (all P < 0.05). The clinical efficacy of the treatment group was better than that of the control group, with statistical significance (P < 0.05). Conclusion Both treatments can effectively reduce the pain and restore the function of the lumbar and leg in patients with lsodorsal myofasciitis, thus improving the quality of life. Ultrasound-guided akupotomye treatment of thoracolumbar fascitis is more effective than ordinary acupuncture in improving the clinical symptoms, which is worthy of further clinical application.

Objective To explore the effect of Bining Decoction combined with akupotomye in the treatment of gouty nephropathy (GN) arthritis with syndrome of accumulation and binding of damp-heat. Methods Sixty patients with GN with syndrome of accumulation and binding of damp-heat admitted to Department of Rheumatology, the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from December 2018 to December 2019 were collected. Through the random number table method, they were divided into treatment group and control group (30 cases in each group). The patients in the treatment group were treated by Bining Decoction combined with akupotomye. The patients in the control group were treated by Febuxostat Tablets. The treatment course for both groups was seven days. After treatment, blood uric acid (BUA), C-reactive protein (CRP), 24 h urine protein, visual analog scale (VAS), and joint activity score were used as the indicators of efficacy observation in the two groups. Results After treatment, the quantitative levels of BUA, CRP, 24 h urine protein, VAS and joint activity score in both groups were lower than those before the treatment, and the quantitative levels of BUA, CRP, 24 h urine protein, VAS and joint activity scores in the treatment group were lower than those in the control group, with statistical significant differences (P < 0.05). Conclusion Bining Decoction combined with akupotomye in the treatment of GN with syndrome of accumulation and binding of damp-heat can effectively reduce BUA, CRP 24 h urine protein, relieve pain and improve joint function.
