The microscopical indentification and determination of the content of five main components of Tagetes erecta L.

Objective To provide theoretical basis for quality control and rational and effective development and utilization of Tagetes erecta L. by studying the microscopical characteristics of root, stem and leaf of marigold by means of microscopic identification, and detecting the contents of main components in different parts. Methods The microstructure features of Tagetes erecta L. were observed by microscopic identification. YMC C18 column (4.6 mm × 250 mm, 5 μm) was used as the column, acetonitrile (B) - 0.1% formic acid aqueous solution (A) was used as mobile phase of gradient elution with the detection wavelength of 257 nm and the colomn temperature was room temperature. The flow rate was 1.0 mL/min. The injection volume of the sample was 10 μL. Results 5,7,3′-trihydroxyl-3,6,4′-trimethoxylflavone (1), syringic acid (2), patulitrin (3), 5,3′-dihydroxyl-3,6,4′-trimethoxylflavone-7-O-β-D-glucopyranoside (4), 5-hydroxymethylfurfuryl methyl succinate (5) five kinds of linear regression equations were: Y = 2.7957X-0.1385, Y = 2.7951X-0.1163, Y = 0.1740X-0.0231, Y = 0.9006X-0.1242, Y = 0.1815X-0.0916. The correlation coefficient of the compounds was 0.099 92-0.099 95, the recovery rate of the sample was 96.43%-101.17%, and the RSD was 0.13%-1.75%. These five chemical components were found in the rhizome of Tagetes erecta L., and the contents of the other four compounds in the stem were larger than those in the root except for the compound 5. The content of compound 5 was the highest in the stem of Tagetes erecta L., up to 2.5%. Conclusion In this experiment, the effective microscopic identification of the roots, stems and leaves of marigold and the determination of five important components are carried out for the first time. The method is simple, accurate and reproducible, and provides a theoretical basis for the quality control of Tagetes erecta L..

Study on the synthesis of novel Celecoxib derivatives and the cardiovascular risk

Objective To reduce the cardiovascular side effects of Celecoxib by modification structure. Methods Four novel derivatives of Celecoxib were synthesized via substitutions of Celecoxib-CF3 group. The structures were identified and determined by the data of LC-MS and NMR spectrum. A total of 36 Wistar rats with foot swelling induced by carrageen glue were randomly divided into 6 groups according to the weight, Celecoxib group (lavage given Celecoxib), compounds 1, 2, 3, 4 group (gastric gavage compounds 1-4) (a daily dose of 5 mg/kg, drug dispersed to 0.5 mL carboxymethyl cellulose sodium) , the control group (0.5 mL daily to sodium carboxy methyl cellulose) aqueous solution. Serum levels of cyclooxygenase-2 (COX-2) and inflammatory factor interleukin-6 (IL-6) were determined by enzyme-linked immunoadsorption assay (ELISA) on 3 days after treatment, and serum levels of clotting factors were determined on 7 days after treatment. Results Compared with Celecoxib group, there was no significant difference in COX-2 levels in the compound 1 group (P > 0.05), while COX-2 levels in compound 2 to 4 group were increased (P < 0.05). Compared with the Celecoxib group, there were no significant differences in IL-6 levels in the compounds 1 to 4 group (P > 0.05). The clotting time of compounds 2 to 4 group was longer than that of celecoxib group (P < 0.05). Conclusion Compounds 3 and 4 have similar effects to Celecoxib in inhibiting the inflammatory factor IL-6, however there is no significant effect on the clotting time of rats. Therefore, compounds 3 and 4 are expected to be Celecoxib-derived drug candidates with low cardiovascular risk.

Effect and safety analysis of Benzbromarone combined with low dose of Colchicine in the treatment of gout

Objective To investigate the effect and safety analysis of Benzbromarone combined with low dose of Colchicine in the treatment of gout. Methods A total of 90 gout patients in Huizhou Third People′s Hospital of Guangdong Province from March 2016 to March 2019 were divided into control group (45 cases) and research group (45 cases) by random number table method. The control group was given routine treatment combined with Benzbromarone, while the research group was given low dose of Colchicine maintenance treatment on the basis of the control group. The clinical efficiency of the two groups was observed, and the visual analogue scale (VAS) scores of the two groups were compared before and after treatment. The levels of uric acid, erythrocyte sedimentation rate, creatinine, serum nitric oxide (NO), endothelin-1 (ET-1), amyloid protein (SAA) and high sensitive C-reactive protein (hs-CRP) were measured before and after treatment. The number of acute episodes and adverse reactions of gastrointestinal tract in the two groups were counted. Results The total effective rate of the research group was higher than that of the control group (P < 0.05). After treatment, the VAS scores of the two groups were decreased, and the research group was lower than that of the control group (P < 0.05). The number of pain episodes in the research group was significantly lower than that in the control group (P < 0.05). After treatment, the levels of uric acid, erythrocyte sedimentation rate and serum ET-1, SAA, hs-CRP were decreased, and the level of serum NO was increased. The level of uric acid, erythrocyte sedimentation rate and serum ET-1, SAA, hs-CRP in the research group were lower than those in the control group, and the level of serum NO was higher than that in the control group (P < 0.05). The incidence of gastrointestinal adverse reactions in the research group was significantly lower than that in the control group (P < 0.05). Conclusion Benzbromarone combined with low dose of Colchicine can improve the clinical efficacy of gout patients, improve pain symptoms and vascular endothelial function, inhibit inflammatory response, decrease the number of acute episodes, and reduce the level of serum uric acid, and alleviate gastrointestinal adverse reactions.

Realationship study on miRNA-675-5p and epithelial-mesenchymal transition in esop-hageal squamous cell carcinoma

Objective To investigated the correlation between the expression of miRNA-675-5p and epithelial-mesenchymal transition (EMT) in esophageal squamous cell carcinoma (ESCC). Methods Real-time quantitative PCR was used to detect the differential expression of miRNA-675-5p in ESCC and normal adjacent tissues, while the correlation between its expression level and clinical stage of patients was analyzed. Liposomes was transfected into ECA109 cell, and divided into miR-675-recursor group, miR-675-5p-inhibition group and NC group. The expression of miRNA-675-5p was up-regulated and inhibited, while the effect of changing the expression of the target gene on the migration and invasion of ECA109 cell lines was analyzed. The expression levels of E-cadherin, N-cadherin, and Vimentin in EMT-associated proteins in each group of cells were detected by western blot experiments, while the effect on the EMT process of ECA109 strain was evaluated. Results Compared with normal adjacent tissues, miRNA-675-5p expression was significantly increased in ESCC tissues (P < 0.05), and comparison of miRNA-675-5p expression in patients with different stages, the differences were statistically significant (P < 0.05). Compared with the NC group, the miR-675-recursor group had significantly enhanced cell migration and invasion ability, and the miR-675-5p-inhibition group had significantly reduced cell migration and invasion ability (P < 0.05). In addition, compared with the NC group, the expression of E-cadherin protein in the miR-675-recursor group was reduced, while the expression of N-cadherin and Vimentin proteins were increased; the expression of E-cadherin protein in the miR-675-5p-inhibition group was increased, while the expression of N-cadherin and Vimentin proteins were reduced (P < 0.05). Conclusion The expression of miRNA-675-5p is related to the TNM stage of ESCC patients. miR-675-5p can promote the interstitial transformation of ESCC cells and thus enhance the invasion ability; down-regulated expression can reverse the EMT process of ESCC cells and reduce the migration and invasion ability.

Study on fingerprint of Herba Rhodiolae in Hongba township of Sichuan by HPLC

Objective To establish high performance liquid chromatography (HPLC) fingerprint of Herba Rhodiolae, and to provide the basis for the establishment of quality standards, and at the same time, to study the quality of different batches of Herba Rhodiolae. Methods RP-HPLC method was used. Waters Xbridge?誖 Shield RP18 (5 μm, 4.6 mm×250 mm) wan used as the column, acetonitrile-water containing 0.1% acetic acid was used as the mobile phase of the gradient elute, with the detection wavelength of 276 nm and the colomn temperature maintained at 30℃. The flow rate was 1.0 mL/min. The recording time was 70 min. Results The HPLC fingerprint method validation met the technical standards. A total of 17 common peaks of Herba Rhodiolae. were found. A total of 12 characteristic peak were identified. They were salidroside, callic acid, caffeic aci, tyrosol, p-coumaric acid, protocatechuic acid, catechin, vanillin, isoquercitrin, quercitrin, chrysophanol 8-O-β-D-glucoside, kaempferol, respectively. The similarity of 13 differernt batches of Herba Rhodiolae. was among 0.863-0.984. Conclusion The specific HPLC fingerprint can be used as a method of scientific evaluation for Herba Rhodiolae. crenulata.

Research progress on the treatment of peritoneal metastasis of gastric cancer

Peritoneal metastasis of gastric cancer has high incidence and short survival time, which is one of the main causes of death in advanced gastric cancer. In spite of several advances have been made in diagnosis and treatment, the prognosis of patients with peritoneal metastasis is still lower. Effective preventive measures are as important as treatment of peritoneal metastasis. Hyperthemic intraperitoneal perfusion chemotherapy, extensive intraoperative peritoneal lavage, early postoperative intraperitoneal chemotherapy and postoperative adjuvant chemotherapy can improve the prognosis of patients with peritoneal metastasis in high-risk groups. The systemic chemotherapy is the main treatment for peritoneal metastasis of gastric cancer, which can be combined with a variety of treatment methods, including intraperitoneal chemotherapy, targeted therapy and immunotherapy. In particular, the conversion therapy can significantly improve the prognosis of gastric cancer patients with peritoneal metastasis. However, the selection of the groups, the regimens and the timing of the conversion therapy are still the current focus and difficulty. This article summarizes the research status of treatment for peritoneal metastasis of gastric cancer in recent years, thus providing some references for the clinical practice.

Research progress on the attenuation and synergism of compatibility of Aconiti Radix and Paeoniae Radix Alba

Aconiti Radix and Paeoniae Radix Alba are traditional couplet medicines for the treatment of wind-cold-dampness arthralgia syndrome. In recent years, researchers have carried out a lot of research work on the the attenuation and synergism of compatibility of Aconiti Radix and Paeoniae Radix Alba. It is found that the compatibility of Aconiti Radix and Paeoniae Radix Alba can reduce the content of diester-alkaloids, inhibit the absorption of diester-alkaloids, and enhance the absorption of paeoniflorin. In addition, the compatibility of the two herbs can effectively reduce the effect of Aconiti Radix on liver drug metabolic enzyme activity, and also regulate the expression level of small molecule metabolites in vivo. This paper reviews the attenuation and synergism of compatibility of Aconiti Radix and Paeoniae Radix Alba from four aspects of clinical application, chemical composition, pharmacodynamics and toxicity evaluation, so as to provide evidence for the clinical safety of the compatibility of Aconiti Radix and Paeoniae Radix Alba and provide a reference for the development of related drugs.

Research progress of aldehyde dehydrogenase 2 in Alzheimer′s disease

Alzheimer′s disease (AD) is a chronic, progressive neurodegenerative disease, and oxidative stress injury and toxic aldehydes deposition play an important role in AD pathology. Mitochondrial aldehyde dehydrogenase 2 (ALDH2) is an aldehyde oxidation-reduction enzyme. ALDH2 activity is determined by its genotype. Carriers of ALDH2 mutation have a higher risk of suffering AD. Basic studies suggest that ALDH2 can reduce the deposition of 4-HNE produced by oxidative stress in important organs, especially brain tissues, thereby reducing the oxidative stress injury of neuronal cells. This article will review the research progress of ALDH2 in AD, and provide novel insights for disease prevention and control.

Research progress of PD-1/PD-L1 inhibitors for the treatment of advanced bladder cancer

Bladder cancer is a common cancer in the urinary system. The current common treatments include surgery and chemotherapy. However, the efficacy of patients with advanced bladder cancer is still unsatisfactory. In recent years, with the advent of immunotherapy represented by immunological checkpoint inhibitors such as programmed cell death 1 (PD-1)/programmed cell death ligation 1 (PD-L1) inhibitors, new treatments for advanced bladder cancer have emerged. PD-1/PD-L1 inhibitors block PD-1/PD-L1 binding, thereby blocking negative immune regulatory signals, then enhancing anti-tumor immune activity. This article will review the principles, current status and adverse effects of PD-1/PD-L1 inhibitors in the treatment of advanced bladder cancer through analyzing data and results in clinical trials, and provide assistance for the clinical treatment of patients with advanced bladder cancer.

Severe fever with thrombocytopenia syndrome (SFTS) is a infectious disease caused by the Phlebovirus belonging to the Bunyaviridae family (also known as SFTS virus). The disease has been reported in China, South Korea and Japan. However there is no credible clinical treatment for this disease. Therefore, early diagnosis of the disease is particularly important to prevent viral transmission and improve patient survival. This article mainly summarizes the diagnosis, prevention and treatment of the disease, and provides reference for subsequent research.

Advancesin research on the relationship between human papilloma-virus infection and genital lesions

Human papillomavirus (HPV) can be divided into high-risk type and low-risk type according to pathogenicity. The high-risk type mainly causes cervical cancer in women and the penis cancer in men, while the low-risk type mainly causes genital warts. Different genotypes cause certain differences in disease types. HPV-16 is the main type of female squamous cell carcinoma; HPV-18 is the main type of female adenocarcinoma and adenosquamous carcinoma; HPV-16 and HPV-18 infection are the main types of male penile cancer; the main types of female genital warts and male genital warts and infertility are HPV-6 and HPV-11. Currently, there are two preventive and therapeutic vaccines for HPV on the market. Different populations should choose vaccines in a targeted and reasonable manner, which is essential for preventing and eliminating HPV.

In view of the high invasiveness and complexity of GBM, there is an urgent to conduct in-depth clinical studies on non-invasive biomarkers that can be used for prognosis and prediction, especially for early diagnosis.

Application of scenario simulation teaching in the improvement of doctor-patients communication ability in surgical interns

Objective To explore the application effect of scenario simulation teaching in the training of interns′ doctor-patients communication ability. Methods Sixty-seven interns who rotated in the Department of General Surgery of Xuanwu Hospital, Capital Medical University from January to December 2018 were selected as the research objects. The 32 interns who participated in the training from January to June 2018 were taken as the control group, and traditional classroom teaching was adopted. A total of 35 interns who participated in the training from July to December 2018 were selected as the experimental group, and scenario simulation teaching was adopted. Liverpool communication skills assessment scale (LCSAS) was used to compare the assessment scores before and after training, and the approval degree of interns for scenario simulation teaching and traditional teaching method was investigated. Results After training, the LCSAS scores of the two groups were significantly higher than those before training (P < 0.05), and the LCSAS score of the experimental group after training was higher than that of the control group, the difference was statistically significant (P < 0.05). The questionnaire showed that there was no statistically significant difference of the ratio of typical and practical cases between the two groups (P > 0.05), and the proportion of novelty and vividness, mobilizing learning interest, enhancing career confidence, promoting clinical thinking ability, improving strain capacity and improving communication ability in the experimental group was significantly higher than the control group, the differences were statistically significant (P < 0.05). Conclusion Compared with the traditional teaching method, the scenario simulation teaching can effectively improve the interns′ doctor-patients communication ability, and lay a solid foundation for the interns to grow up into qualified clinicians.

As the key point of education shifts from theory to practice and the role of medical student is converted from a student to a doctor, how to cultivate autonomous learning ability is an important task in clinical teaching of standardized training for residents. Aiming at the current situation and characteristics of autonomous learning ability of resident doctors, this paper analyzes the importance and necessity of autonomous learning ability in the clinical teaching. Meanwhile, this paper discusses how to improve the autonomous learning ability of resident doctors from the following four aspects, including the construction of the teaching mode of autonomous learning ability, cultivation of consciousness of autonomous learning, the importance of reflection in time and auxiliary teaching based on WeChat platform.

Application effect of holistic integrated medical teaching method in teaching mode of traditional Chinese medicine gynecology

Objective To explore the application effect of holistic integrated medical teaching method in teaching mode of traditional Chinese medicine gynecology. Methods From March to July 2019, 2 natural classes of students (112 students in total) majoring in traditional Chinese medicine of grade 2017 from Hubei College of Chinese Medicine were selected and randomly divided into the experiment group (56 cases) and the control group (56 cases) by lottery. The experiment group was given holistic integrated medical teaching mode, while the control group was given traditional teaching mode. Both groups were taught by the same teacher. After teaching, the skill assessment and theoretical knowledge examination were used to evaluate evaluation results, and questionnaire was used to investigate the satisfaction of teaching method and teaching effect. Results The average scores of skill assessment and theoretical knowledge examination in the experiment group were higher than those in the control group, the differences were statistically significant (P < 0.05). There was no significant difference of the satisfaction of teaching method and teaching effect between the two groups (P > 0.05). Conclusion The holistic integrated medical teaching method can improve the teaching quality of traditional Chinese medicine gynecology, inspire the initiative to learn actively, practice clinical thinking, advance the ability of transforming theory into practice.

Application of microteaching in the training of standardized general thinking model

Objective To explore the application of microteaching in the training of standardized thinking model of general practice, and to provide theoretical basis for the selection of training mode general practitioners. Methods From January 2017 to December 2018, 120 general practitioners who received standardized general practice thinking model training in Health Capacity Building and Continuing Education Center of Shenzhen City were selected as the subjects of this study. They were divided into the observation group and the control group by random number table method, with 60 general practitioners in each group. General practitioners in the control group adopted the traditional general practice training mode, while general practitioners in the observation group adopted the microteaching mode. After the training, the general practitioners of the two groups all received objective structrual clinical examination (OSCE) based on the unified rating scale. The examination results of the two groups were compared, and a questionnaire survey was conducted on professional achievement, self-confidence and communication ability. Results Before the intervention, there was no statistically significant difference between the two groups in examination results, professional achievement, self-confidence and communication skills (P > 0.05). After intervention, the above indicators in the observation group were higher than those in the observation group before intervention and the control group, and the examination result of the control group was higher than that before intervention, with statistically significant differences (all P < 0.05). Conclusion Microteaching model can stimulate the learning interest of general practitioners, improve the comprehensive ability of general practitioners, and effectively improve the quality of general practitioner training.

A meta-analysis of the clinical efficacy and safety of hyperbaric oxygen in the treatment of diabetic peripheral neuropathy

Objective To investigate the clinical effect and safety of hyperbaric oxygen in the adjuvant treatment of diabetic peripheral neuropathy. Methods Cochrane, PubMed, EMBase, CNKI, Wanfang and VIP databases were searched to screen randomized controlled trials of hyperbaric oxygenation in the treatment of diabetic peripheral neuropathy reported from the establishment of the database to April 2019. After literature screening, data extraction and literature quality evaluation by two researchers, the meta-analysis of effective rate and nerve conduction velocity was taken by RevMan 5.2 software. Results A total of 16 articles including 1644 patients were included, including 836 in the experimental group and 808 in the control group. The results of meta-analysis showed that the total effective rate [OR = 7.97, 95%CI (3.95, 16.11), P < 0.000 01, motor nerve conduction velocity [MDmedian nerve = 5.51, 95%CI (3.78, 7.25), P < 0.000 01; MDulnar nerve = 4.49, 95%CI (2.20, 6.78), P = 0.0001; MD peroneal nerve = 5.67, 95%CI (4.18, 7.16), P < 0.000 01; MDtibial nerve = 3.13, 95%CI(1.22, 5.05), P = 0.001]; sensory nerve conduction velocity [MDmedian nerve = 3.25, 95%CI (1.51, 4.99), P = 0.0003; MDulnar nerve = 7.57, 95%CI (2.50, 12.64), P = 0.003; MDperoneal nerve = 4.42, 95%CI (3.70, 5.14), P < 0.000 01; MDtibial nerve = 2.46, 95%CI(0.64, 4.28), P = 0.008] between the experimenttal group and control group, the differences were statistically significant. There were 8 adverse events and side effects in the experimental group, including mild pain, earache, tinnitus, and dizziness, but the injuries were minor, and none in the control group. Conclusion Hyperbaric oxygen-assisted therapy is a safe and effective method , which can improve the therapeutic efficiency of diabetic peripheral neuropathy patients, increase the nerve conduction velocity, reduce adverse events and side effects, and reduce injury.

Effect of permissive hypercapnia on lung injury induced by tourniquet-related limb ischemic repufusion in elded patients

Objective To investigate the effect of permissive hypercapnia on lung injury induced by tourniquet-related limb ischemic repufusion in elderly patients. Methods Sixty patients were scheduled for total knee replacement from January 2018 to January 2019 in Ward One, Department of Orthopedics of the First Affiliated Hospital of University of Science and Technology of China, Anhui Provincial Hospital were selected, and were randomly divided into the normal ventilation group and permissive hypercapnia group by random number table method, 30 cases in each group. In the normal ventilation group, the partial pressure of carbon dioxide (PaCO2) was maintained at 35 to 45 mmHg, while in the permissive hypercapnia group, PaCO2 was maintained at 60 to 70 mmHg. The arterial blood gas analysis were performed and recorded before the induction of anesthesia (T0), 30 minutes (T1), 6 hours (T2) 24 hours (T3) and 48 hours (T4) after surgery of the two groups. The partial pressure of oxygen (PaO2) and PaCO2 were measured, while the oxygenation index (OI), alveolar-arterial oxygen difference [P(A-a)DO2], and respiratory index (RI) were calculated and recorded. Central venous blood samples were collected at the above time points, and the levels of serum tumor necrosis factor-α (TNF-α), interleukin-10 (IL-10), human complement 3a (C3a) and C-reactive protein (CRP) were determined by enzyme-linked immunosorbent assay (ELISA). Pulmonary complications and hospital stay were recorded at 72 h after surgery in both two groups. Results Compared with T0, the OI of two groups were significantly reduced, the [P(A-a)DO2] and RI were significantly increased between T1-T3 (P < 0.05), the serum TNF-α concentration were significantly reduced, and the serum IL-10 concentration were significantly increased between T1-T4. The concentrations of serum C3a and CRP were increased significantly from T2-T4 (P < 0.05). Compared with the normal ventilation group, the OI of permissive hypercapnia group were significantly increased, while (A-a)DO2 and RI were significantly reduced at T2-T3, and the concentration of serum TNF-a were significantly reduced, while the concentration of serum IL-10 were significantly decreased at T1-T4, the concentrations of serum C3a and CRP were significant decreased at T2-T3, and the hospital stay was significantly shorter (P < 0.05). There were no significant differences in the incidence of complications such as wake-up time, pulmonary acute respiratory distress syndrome (ARDS) at 72 hours postoperatively, and pulmonary infection between the two groups (P > 0.05). There were no atelectasis or pulmonary edema occurred in the two groups. Conclusion Permissive hypercapnia can reduce lung injury caused by ischemia-reperfusion of tourniquet-related limbs in elderly patients, and improve lung oxygenation and diffuse function. The mechanism may be related to the suppression of inflammatory responses.

The value of thromboelastography for the antiplatelet drug reactivity in patients with diabetic nephropathy undergoing maintenance hemodialysis

Objective To explore the value of thromboelastography (TEG) in evaluating the effect of antiplatelet drugs after percutaneous coronary intervention (PCI) in patients with diabetic nephropathy (DN) maintenance hemodialysis. Methods A total of 46 patients with maintenance hemodialysis who were treated at the People′s Hospital of Zhejiang Province from May 2013 to March 2016 and received aspirin combined with clopidogrel within 1 year after PCI were included. According to whether end-stage renal disease caused by DN started maintenance hemodialysis, all patients were divided into DN group (20 cases) and NDN group (26 cases), and TEG test was performed. The maximum thrombus (MA), arachidonic acid (AA) pathway inhibition rate, and adenosine diphosphate (ADP) receptor inhibition rate were used to evaluate drug resistance and analyze clinical significance. Results The MA value in DN group was higher than that in group B, and the difference was statistically significant (P < 0.05). The resistance rate of double platelet aggregation in DN group was higher than that in NDN group, and the difference was statistically significant (P < 0.05). There were no significant differences in the aspirin resistance rate and clopidogrel resistance rate between the two groups of patients (P < 0.05). Conclusion Patients with DN have a higher risk of thrombosis after PCI compared with NDN patient. Both DN and NDN patients have resistance to dual antiplatelet drugs, and patients with DN are very resistant, suggesting that platelet function to guide the adjustment of the drug may be required in such high-risk patients.

Effect of high flux hemodialysis combined with Haikun Shenxi Capsules on oxidative stress and microinflammation in patients with diabetic nephropathy

bjective To investigate the effect of high-flux hemodialysis (HFHD) combined with Haikun Shenxi Capsules on oxidative stress and microinflammation in patients with diabetic nephropathy. Methods A total of 80 patients with diabetic nephropathy who underwent hemodialysis for more than 6 months from June 2016 to May 2019 in People′s Hospital of Yuxi City were enrolled. The patients were divided into the control group and the combination group by random number table method, with 40 cases in each group. The control group was only treated with HFHD, while the combination group was treated with HFHD combined with Haikun Shenxi Capsules. The difference of renal function index [blood urea nitrogen (BUN), serum creatinine (Scr), creatinine clearance (Ccr) and 24 h urine protein quantitation (UAER)], related inflammatory index [Serum levels of interleukin-6 (IL-6), high-sensitivity C-reactive protein (hs-CRP) and tumor necrosis factor-α (TNF-α) and oxidative stress level [malondialdehyde (MDA), glycopeptide peroxidase (GSH-Px) and superoxide dismutase (SOD)] between the two groups were compared before and after treatment. The clinical effect of the two groups was compared after treatment, and the incidence of adverse reactions was analyzed. Results Compared with before treatment, BUN, Scr, and UAER in the two groups after treatment were significantly decreased, Ccr was significantly increased, and serum IL-6, hs-CRP, TNF-α, and MDA levels were significantly decreased, GSH-Px and SOD levels were significantly increased (P < 0.05). The Ccr, GSH-Px and SOD indexes of the combination group were significantly higher than those of the control group, while the BUN, Scr, UAER, IL-6, hs-CRP, TNF-α and MDA levels were significantly lower than those of the control group (P < 0.05). The clinical effective rate in the combination group was significantly better than that of the control group (P < 0.05). There was no significant adverse reaction in both groups. Conclusion The combination of HFHD and Haikun Shenxi Capsules can significantly improve the renal function, relieve inflammatory state and reduce oxidative stress in patients with diabetic nephropathy. The curative effect is better than single HFHD treatment, it is worth to recommended for clinical application.

Effect of combination of Qiangli Loquat Dew and Western medicine on acute exacerbation of chronic bronchitis and its effect on inflammatory factors and quality of life

Objective To investigate the effect of combination of Qiangli Loquat Dew with Western medicine on acute exacerbation of chronic bronchitis and its effect on inflammatory factors and quality of life. Methods From February 2017 to February 2019, 98 cases with acute exacerbation of chronic bronchitis received by the 910th Hospital of Joint Logistic Support Force of the Chinese People′s Liberation Army were selected. The patients were randomly divided into the study group and the control group by random numbers method, with 49 cases in each group. The study group was treated with Qiangli Loquat Dew combined with western medicine, while the control group was treated with western medicine. Clinical efficacy, recovery time of clinical symptoms, inflammatory factors, quality of life and adverse reactions of patients were compared between the two groups. Results The total effective rate of the study group after 2 weeks of treatment was higher than that of the control group, and the difference was statistically significant (P < 0.05). The remission time of cough, sputum, fever and lung ralogue symptoms in the study group was shorter than those in the control group, and the differences were statistically significant (all P < 0.05). The serum levels of interleukin-6 (IL-6), IL-8 and tumor necrosis factor-α (TNF-α) of patients in the two groups decreased after 2 weeks of treatment compared with those before treatment, and the study group were lower than those in the control group, the differences were statistically significant (all P < 0.05). The scores of physiological function, physical pain, role physical, general health, social function, energy, emotional function and mental status of patients in the two groups after 2 weeks of treatment were all higher than those before treatment, and the scores of the study group were higher than those in the control group, the differences were statistically significant (all P < 0.05). There was no significant difference of the incidence of adverse reactions of patients between the two groups (P > 0.05). Conclusion In the treatment of acute exacerbation of chronic bronchitis with combination of Qiangli Loquat Dew and Western medicine, the curative effect is definite, the level of inflammatory factors can be effectively improved, and the quality of life of patients can be improved without increasing the incidence of adverse reactions.

Effect of preoperative concurrent chemoradiotherapy combined with surgery in the treatment of esophageal cancer

Objective To investigate the clinical effect of preoperative concurrent chemoradiotherapy combined with surgery in the treatment of esophageal cancer. Methods From January 2016 to December 2018, 51 patients with esophageal cancer admitted to the Tumor Hospital Affiliated to Xinjiang Medical University were selected and divided into the observation group (n = 27) and the control group (n = 24) according to the random number table method. The observation group received Docetaxel+Cisplatin chemotherapy before surgery, and received three-dimensional conformal radiotherapy simultaneously. Surgery was performed 2 weeks after chemotherapy. The control group received surgical treatment directly. The complete resection, perioperative complications, 1-year, 2-year and 3-year survival rates of patients were compared between the two groups. The occurrence of short-term curative effect and toxic side effect in the observation group was recorded. Results After eliminating the lost follow-up cases, 26 cases remained in the observation group and 22 cases in the control group. The objective remission rate (ORR) and disease control rate (DCR) were 92.31% and 100.00% respectively in the observation group after preoperative concurrent chemoradiotherapy. In the observation group, acute toxic side effects of grade Ⅲ or above included neutropenia in 2 cases, thrombocytopenia in 1 case, nausea/vomiting in 4 cases, alopecia in 5 cases and radiation dermatitis in 1 case. The complete resection rate and 1-year, 2-year and 3-year survival rate in the observation group were significantly higher than those in the control group, with statistically significant differences (P < 0.05). There was no significant difference in the incidence of perioperative complications between the two groups (P > 0.05). Conclusion Preoperative concurrent chemoradiotherapy combined with surgical treatment can improve the survival rate and complete resection rate of patients with in vitro squamous cell carcinoma.

Clinical study on the changes and significance of thermal sensation thresholds in patients with herpes zoster

Objective To measure and quantitative analyze the thermal sensation thresholds of the affected side and healthy side skin of herpes zoster patients,in order to understand the function of primary afferent fibers in the patients with herpes zoster. Methods From December 2015 to February 2017, 80 cases with herpes zoster who admitted to Department of Pain, the First Affiliated Hospital of Chongqing Medical University as research objects. TSA-Ⅱ thermal sensation analyzer was used to detect the cold sensation (CS), warm sensation (WS), cold pain sensation (CP) and heat pain sensation (HP) thresholds by using the method of limits. Statistical analysis was performed and Pearson correlation analysis was used to analyze the correlation between the thermal sensation thresholds of the affected sides of herpes zoster patients and the course of disease, numerical rating scale (NRS), ID pain scores. Results The CS and CP thresholds on the affected side were lower than those on the healthy side, and the HP thresholds were higher than those on the healthy side, and the differences were statistically significant (all P < 0.05). There was no significant difference in the WS thresholds between the two sides (P > 0.05). The CS and CP thresholds of the affected side were negatively correlated with the course of disease (r = -0.399, -0.374, P < 0.05). Conclusion The thermal sensation thresholds testing is one of the objective methods to evaluate the function of C and Aδ primary afferent fibers, which is beneficial to diagnose and evaluate the severity of peripheral nerve damage of the disease.

Effect observation of constrained patrol records in patients with neurosurgical agitation

Objective To investigate the clinical effect of constrained patrol records in patients with neurosurgical agitation. Methods From January 2017 to January 2019,92 cases with brain injury and agitation who underwent Department of Critical Care Medicine, Hai′an Hospital Affiliated to Nantong University were selected. According to the random number table, They were divided into the control group and the observation group, with 46 cases in each group. The control group performs regular constraint management, and the observation group used the constraint patrol records on the basis of the control group. The incidence of constrained complications, constrained complaints rate and families satisfaction were compared. Results The incidence of gastric tube detubation or extubation, the infusion tube detubation or tube extubation, electrical wires detubation or extubation and peripheral circulation obstruction in the observation group were lower than those in the control group, the differences were statistically significant (all P < 0.05). There were no significant differences between the two groups in the incidence of urinary tube detubation or extubation, head drainage tube detubation or extubation, skin injury and falling bed (P > 0.05). The constrained complaints rate of the observation group was lower than that of the control group, and the difference was highly significant (P < 0.01). The families satisfaction in the observation group was higher than that of the control group, and the difference was statistically significant (P < 0.05). Conclusion Applying the constrained patrol records to patients with neurosurgical agitation can effectively reduce the constraint complaints rate, improve the families satisfaction, and reduce the constraint complications. It is worth promoting.

Systematic evaluation and meta-analysis of pain and prognosis after radical gastrectomy under general anesthesia combined with epidural anesthesia and general anesthesia alone

Objective To systematically evaluate the effects of general anesthesia combined with epidural anesthesia and general anesthesia alone on postoperative pain and prognosis of radical gastrectomy, and to conduct meta-analysis. Methods The Cochrane library, PubMed database, MEDLINE database, EMBASE database, Chinese biomedical literature database and CNKI database were searched by computer from the establishment of the database to January 2018. Chinese search words: general anesthesia, epidural anesthesia, gastric cancer, gastric malignancy, gastrectomy, case-control study, cohort study, randomized controlled trial. English search words: epidural anesthesia, general anesthesia, gastric cancer. Literature screening and data extraction were conducted independently by 2 staff members. The results of screening and data extraction shall be checked by the third staff member. If there was any discrepancy, it shall be discussed and agreed upon. Main outcome measures: survival rate, incidence of pulmonary infection and length of hospital stays. Results Eight studies were included, including a total of 5105 patients receiving radical gastrectomy for gastric cancer. 1817 patients receiving general anesthesia combined with epidural anesthesia and 3288 patients receiving general anesthesia alone. Meta-analysis showed that, there was no significant difference in survival rate between the two anesthesia methods (HR = 0.78, 95%CI:0.60, 1.01, P > 0.05). The incidence of pulmonary infection under general anesthesia combined with epidural anesthesia was lower than that under general anesthesia alone(RR = 0.28, 95%CI:0.15, 0.52), and the length of hospital stays were shorter than that under general anesthesia alone (MD = -1.50, 95%CI:-2.62, -0.39), with statistically significant differences (all P < 0.05). Conclusion General anesthesia combined with epidural anesthesia may improve the survival rate of patients after radical gastrectomy and pulmonary infection rate, and shorten hospital stays. The above conclusions may have confounding bias and detection bias, which should be used with caution in clinical practice. More specific conclusions need to be confirmed by a large randomized controlled trial.

Effect of transcutaneous electrical acupoint stimulation combined with Dexmedetomidine on postoperative delirium in elderly patients

Objective To investigate the effect of transcutaneous electrical acupoint stimulation (TEAS) combined with Dexmedetomidine (DEX) on postoperative delirium (POD) in elderly patients. Methods From January 2017 to November 2018, 96 cases with elderly patients underwent pedicle screw internal fixation under elective general anesthesia in Beijing Hospital of Traditional Chinese Medicine, Capital Medical University were selected. According to the random number table method, they were devided into DEX group (group D) and combination group (group T), with 48 cases in each group. Group D was used DEX before anesthesia induction, while group T was used TEAS in combination on the basis of group D, and both groups stopped the above treatment 30 min before the end of the operation. The incidence of POD was compared within 5 d after operation. The does of Propofol was compared. Neuron-specific enolase (NSE), serum tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and superoxide dismutase (SOD) levels in the two groups were compared within 1 d before operation and 1, 5 d after operation. Results There was no significant difference in POD incidence between the two groups (P > 0.05). The dose of Propofol in group T were lower than that in group D, and the differences were statistically significant (P < 0.05). In group D, the levels of NSE, TNF-α, and IL-6 on 1, 5 d after operation were higher than those on 1 d before operation, while the levels of SOD were lower than that on 1 d before operation, with statistically significant differences (all P < 0.05). The levels of NSE, TNF-α, and IL-6 on 5 d after operation were lower than those on 1 d after operation, while the levels of SOD were higher than those on 1 d after operation, with statistically significant differences (all P < 0.05). In group T, the levels of NSE, TNF-α and IL-6 on 1 d after operation were higher than those on 1 d before operation, while the levels of SOD were lower than those on 1 d before operation, with statistically significant differences (all P < 0.05). The levels of IL-6 on 5 d after operation were higher than those on 1 d before operation, and the differences were statistically significant. The levels of NSE, TNF-α, and SOD on 5d after operation were not statistically significant compared with those on 1 d before operation (all P > 0.05). The levels of NSE, TNF-α, and IL-6 on 5 d after operation were lower than those 1 d after operation, while the levels of SOD were higher than those 1 d after operation, with statistically significant differences (all P < 0.05). Conclusion TEAS combined with DEX do not reduce the incidence of POD in elderly patients, but can reduce intraoperative Propofol dosages, inhibit inflammatory response and reduce brain injury.

Effect of lumbar stiffness combined block for labor analgesia on brain natriuretic peptide and high sensitivity troponin T in preeclampsia pregnant women during delivery and postpartum

Objective To observe the effect of lumbar stiffness combined block for labor analgesia on brain natriuretic peptide (BNP) and high sensitivity troponin T (cTnT-hs) in preeclampsia pregnant women during delivery and postpartum. Methods From January 2017 to December 2018, 120 cases with preeclampsia pregnant women who were hospitalized in Department of Anesthesiology, Nanning Maternal and Child Health Hospital, Guangxi Zhuang Autonomous Region were selected as research object. According to the random number table methcd, they were divided into observation group and control group, with 60 cases in each group. Control group was treated with routine spasmolysis, antihypertensive, while observation group was treated with lumbar stiffness combined block for labor analgesia on the basis of control group. Natural childbirth pregnant women of observation group were recorded as the observation subgroup, natural childbirth pregnant women of control group were recorded as the control subgroup. BNP and cTnT-hs levels were compared in the two subgroups during the incubation period, the active period, the second stage of labor, 2 h after delivery and the 42nd day after delivery, and the occurrence of blood pressure on the 42nd day after delivery were recorded in the two subgroups. Results There were 45 natural childbirth pregnant women in the observation group and 39 natural childbirth pregnant women in the control group. BNP levels at different stages of labor in the observation subgroup were lower than those in the control subgroup, and the differences were statistically significant (all P < 0.05). There were no significant differences in cTnT-hs between the two subgroups at different stages of labor (P > 0.05). Conclusion Lumbar stiffness combined block for labor analgesia can reduce the delivery process and post-delivery BNP level of pre-eclampsia pregnant women, reduce the incidence of hypertension on the 42nd day after delivery, and reduce the rate of cesarean section.

Diagnosis and treatment of ulcerative colitis from the theory of “removing stagnation and invigorating spleen”

“Spleen deficiency and dampness stagnation” is the main pathogenesis of ulcerative colitis. “Stagnation” includes two pathological products, damp stagnation and stagnation of qi and blood, which interact with each other in the pathogenic process. First use the method of “clearing away dampness and stagnation” to dispel the pathogenic factors of cold-dampness, dampness heat and phlegm turbidity, so as to make qi and blood unblocked. “Invigorating qi and strengthening spleen” is the key to the treatment. Emphasis should be laid on the deficiency of spleen, and the foundation should be stabilized so as to make qi and blood come into being. The soil in the five elements is weak, and the relatively strong wood will suppress the soil. Therefore, the treatment of soothing the liver can make qi smooth. In clinic, nourishing blood and tonifying deficiency, promoting blood circulation and stopping bleeding are added to ventilate blood stagnation. Long course of disease often injures yang in kidney. Some Chinese herbs for tonifying kidney yang can be added. Shenling Baizhu Powder and Gegen Qinlian Decoction are modified to relieve stagnation and invigorate the spleen. Both qi and blood are regulated. Liver and kidney are also regulated to dispel pathogenic factors without damaging the positive qi. It provides an important idea for alleviating clinical symptoms and preventing recurrence.

Effect of Chinese medicine starvation-free fasting therapy on glucolipid metabolism and liver fat deposition in patients with nonalcoholic fatty liver disease

Objective To study the efficacy of Chinese medicine starvation-free fasting therapy on nonalcoholic fatty liver disease (NAFLD) and its effects on glucolipid metabolism and liver fat deposition. Methods A total of 124 inpatients with NAFLD from October 2018 to March 2019 in Shaanxi Provincial Hospital of Chinese Medicine were selected and divided into two groups according to the random number table method, with 62 cases in each group. Fasting group was treated with Chinese medicine starvation-free fasting therapy, and routine group was given Western medicine, and two groups were treated for 2 weeks. The clinical efficacy, Chinese medicine syndrome scores, body surface measurement indicators, glucolipid metabolism and liver fat deposition were compared between two groups, and the occurrence of adverse reactions was observed. Results The clinical efficacy in fasting group was significantly better than that in routine group (P < 0.05). After treatment, Chinese medicine syndrome scores in two groups were significantly reduced (P < 0.05), and the scores in fasting group were lower than those in routine group (P < 0.05). After treatment, fasting blood-glucose, total cholesterol and triacylglycerol were significantly decreased in two groups (P < 0.05), which in fasting group were lower than those in routine group (P < 0.05). After treatment, controlled attenuation parameter, liver stiffness measurement and liver fat content in two groups were significantly decreased (P < 0.05), which in fasting group were lower than those in routine group (P < 0.05). Conclusion Chinese medicine starvation-free fasting therapy for NAFLD is beneficial to alleviate the symptoms and weight of patients, improve glucolipid metabolism and reduce liver fat deposition. And its clinical efficacy is superior to routine western medicine.

The effect of Huazhuo Jiedu Hewei Recipe on VEGF, EGF and Th1/Th2 balance in patients with chronic atrophic gastritis

Objective To observe the effect of Huazhuo Jiedu Hewei Recipe on vascular endothelial growth factor (VEGF), epidermal growth factor (EGF) and Th1/Th2 balance in patients with chronic atrophic gastritis with turbid toxin accumulation syndrome. Methods From March 2017 to December 2018, 60 patients with chronic atrophic gastritis diagnosed by gastroscopy in Department of Spleen and Stomach Diseases of Hebei Hospital of Traditional Chinese Medicine were divided into control group and treatment group according to the random number table method, with 30 cases in each group. Control group was treated with Wefuchun, and treatment group was treated with Huazhuo Jiedu Hewei Recipe for 12 weeks. The main symptoms, Th1/Th2 cell level, serum VEGF and EGF levels were compared between two groups. ELISA kit was used to detect the expression of Th1 cytokine IFN-γ and Th2 cytokine serum IL-4, VEGF and EGF. Results Clinical effect of treatment group was better than that of control group, the difference was statistically significant (P < 0.05). After treatment, the scores of stomachache, distention and anorexia in two groups were lower than those before treatment, and scores in treatment group were lower than those in control group, the differences were statistically significant (P < 0.05). There was no significant difference in serum IFN-γ levels between two groups before and after treatment (P > 0.05). After treatment, the level of IL-4 in two groups was higher than that before treatment, and the level of IL-4 in treatment group was higher than that in control group, the difference was statistically significant (P < 0.05). After treatment, the levels of VEGF and EGF in two groups were lower than those before treatment, the differences were statistically significant (P < 0.05). There was no significant difference in levels of VEGF and EGF between two groups (P > 0.05). Conclusion Huazhuo Jiedu Hewei Recipe can not only improve clinical symptoms of chronic atrophic gastritis, but also regulate the balance of Th1/Th2, down-regulate the expression of VEGF and EGF and play a therapeutic role.

Experience of Li Shuanlu′s two-step method of tranquilizing the mind and activating spleen in the treatment of advanced gastric malignant tumors

As the treatment of advanced malignant gastric tumor is intractable, the chief physician Li Shuanlu considers anxiety and depression as the main symptoms in the primary survey. Mainly spleen deficiency, the pathogenesis is simultaneous insufficiency and excess, which should be treated through recuperating the spirit and regulating the spleen and stomach step by step. The primary survey uses the treatments of suggestopedia, meditation music, and baduanjin exercise to recuperate the spirit, achieving the purpose of ataraxy, improving the bad mood, enhancing the immunity, and establishing confidence in the treatment. Complying with the saying “when the diseases are not cured, start from the spleen and stomach”, the subsequent diagnosis focuses on ameliorating the functioning of the spleen and stomach for which is an important therapy throughout the treatment of advanced malignant gastric tumor. The method of activating the spleen usually chooses Liujunzi Decoction and put a heavy dose on Agrimonia pilosa and Oldenlandia diffusa. Through the above treatments, the clinical efficacy is positive.

Study on the similarities and differences of the effectiveness criteria of acupuncture in periarthritis of shoulder

Objective To find out the common criteria/scales for evaluating the curative effect of acupuncture on periarthritis of shoulder in China, to sort out the contents concerned by different criteria/scales, and to find out the common content for the curative effectiveness for statement the core index set of curative effectiveness of acupuncture on periarthritis of shoulder. Methods By using the method of literature research, the articles were published in CNKI and Wanfang database from January 2014 to January 2019. The keywords were “acupuncture” combined “periarthritis of shoulder/shoulder coagulation/shoulder leakage/shoulder arthralgia/fifty shoulders/frozen shoulder”. Finally, 65 articles were included. Literature metrological analysis was carried out around the criteria and contents of efficacy evaluation, main outcome and measurement tools, secondary outcome and measurement tools of acupuncture treatment for periarthritis of shoulder. Results The standard/scale of commonly used acupuncture therapy for periarthritis of shoulder was not uniform. The the main criterion was effective rate and pain. There were different criteria for different effectiveness and different criteria for the same effectiveness. Existing evaluation criteria/scales pay more attention to pain, activity and functional dimensions. Conclusion The criteria/scales for the effectiveness of acupuncture in the treatment of periarthritis of shoulder are not uniform, and pain, activity and function are the concerned dimensions. Acupuncture treatment of periarthritis of shoulder should reflect the advantages of Chinese medicine holistic treatment and focus on the long-term effect.

Application value of diffusion weighted imaging eADC value in differential diagnosis of benign and malignant prostatic nodules

Objective To investigate the feasibility of differential diagnosis of benign and malignant prostate nodules by the exponential apparent dispersion coefficient (eADC) of magnetic resonance imaging (MRI) diffusion weighted imaging (DWI). Methods From November 2016 to July 2019, 50 prostate cancer patients and 50 benign prostatic hyperplasia patients were selected for MRI DWI examination in the Second Affiliated Hospital of Bengbu Medical College, all of which were confirmed by pathology. DWI signal, apparent diffusion coefficient (ADC) value and eADC value of benign and malignant prostate lesions were compared and analyzed. The receiver operating characteristic curve (ROC) was used to obtain the optimal diagnostic cut-off points of ADC and eADC. Results In the 50 patients with benign prostatic hyperplasia, the hyperplasia nodules were located in the central zone and the transitional zone, and the T2 lipid pressure presented heterogeneous mixed signals with unclear boundaries. DWI showed equal or slightly higher signals, ADC image showed slightly higher signals, and eADC image showed low signals. Among the 50 patients with prostate cancer, cancer nodules were located in the peripheral zone in 12 cases, central zone in 7 cases, translobular growth in 5 cases, multiple nodules in 6 cases, diffuse growth in 20 cases, bone metastasis in 2 cases, bone metastasis plus lymph node metastasis in 1 case. The T2 lipid pressure of the cancer nodule presented low or slightly low signal, most of the boundary was clear, DWI showed high or slightly high signal, ADC image showed significantly low signal, and eADC image showed high signal. The mean ADC image value of prostate cancer was lower than that of benign prostatic hyperplasia, and the difference was highly significant (P < 0.01). ADC value was 1.245×10-3 mm2/s as the best diagnostic threshold, with a sensitivity of 94.6% and specificity of 90.0%. The mean value of eADC of prostate cancer was higher than that of benign prostatic hyperplasia, and the difference was highly significant (P < 0.01). The threshold value of eADC was 0.395, with a sensitivity of 95.2% and specificity of 91.5%. The diagnostic efficacy of ADC and eADC was close. Conclusion The eADC image has similar efficacy with ADC image, eliminates the T2 transmission effect and improves tissue contrast, making it a more valuable reference for the evaluation of prostate cancer.

Effect of Suhuang Zhike Capsules combined with Montelukast in the treatment of patients with stable chronic obstructive pulmonary disease

Objective To study the effect of Suhuang Zhike Capsules combined with Montelukast in the treatment of patients with stable chronic obstructive pulmonary disease (COPD). Methods From June 2017 to January 2019, 146 patients with stable COPD treated in Puren Hospital Affiliated to Wuhan University of Science and Technology were recorded as observation objects. The patients were divided into observation group and control group according to random number table method, with 73 cases in each group. The two groups were treated with routine treatment, on basis of which, the patients in the control group were treated with Montelukast, and the observation group was treated with Suhuang Zhike Capsules on basis of the control group. The two groups were treated for 3 months, and the efficacy, St george′s respiratory questionnaire (SGRQ) scores, pulmonary function, inflammatory factors and adverse reactions of the drugs were compared between the two groups. Results The total effective rate of the observation group was significantly higher than that of the control group (P < 0.05). After treatment, the SGRQ scores in the two groups were significantly lower than those before treatment, and the observation group was significantly lower than that of the control group (P < 0.05). After treatment, the forced expiratory volume in the first second (FEV1), FEV1/maximal vital capacity (FEV1/FVC), maximal ventilation volume (MVV) and residual volume/total lung volume (RV/TLC) in the two groups were significantly higher than those before treatment, which of the observation group were significantly higher than the control group (P < 0.05). After treatment, C-reactive protein (CRP), interleukin (IL)-17, IL-8 and tumor necrosis factor-α (TNF-α) in the two groups were significantly lower than those before treatment, which of the observation group were significantly lower than the control group (P < 0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Suhuang Zhike Capsules combined with Montelukast is effective in the treatment of stable COPD patients, and can improve the pulmonary function and inflammatory factor level of the patients, and the safety is also good, which is worth promoting.

Relationship between high mobility group protein B1 level and helper T cell 17/regulatory T cell balance in patients with primary Sj?觟gren′s syndrome

Objective To investigate the relationship between the level of high mobility group box 1 (HMGB1) and the balance of helper T cell 17 (Th17) /regulatory T cells (Treg) in peripheral blood of patients with primary Sjögren′s syndrome (pSS). Methods Forty patients with pSS in Rheumatology Department of the Second Hospital of Shanxi Medical University from May 2018 to May 2019 were selected as pSS group, and 20 healthy subjects were control group. Elbow venous blood was collected. The number of Th17 and Treg cells was detected by flow cytometry, and Th17/Treg ratio was calculated. Cytometric bead array was used to detect serum interleukin (IL)-17, IL-6 and IL-10 levels. Enzyme-linked immunosorbent was used to detect the levels of HMGB1 and transforming growth factor-β (TGF-β). Each of the above indicators was compared between two groups, and the correlation between HMGB1 and the expression of various cells and cytokines was analyzed. Results The levels of HMGB1, Th17 and Th17/Treg in pSS group were higher than those in control group, while Treg was lower than that in control group, and the difference was statistically significant (P < 0.05). The levels of IL-17, IL-6 and TGF-β in pSS group were higher than those in control group, while IL-10 was lower than that in control group, the differences were statistically significant (P < 0.05). Correlation analysis showed that HMGB1 was positively correlated with Th17, Th17/Treg, IL-17 and IL-6 levels (r = 0.738, 0.502, 0.762, 0.578, all P < 0.05). There was a negative correlation between the levels of HMGB1 and Treg, IL-10 (r = -0.657, -0.686, all P < 0.05), no significant correlation with TGF-β (P > 0.05). Conclusion HMGB1 in peripheral blood of patients with pSS is abnormally high, and there is obvious T cell immune abnormality compared with healthy people, suggesting that HMGB1 may participate in the development of pSS by regulating Th17/Treg balance.

Objective The research trend of diabetes care in recent ten years is presented to provide reference for related domestic research. Methods The retrieval period is from January 1, 2009 to December 31, 2018, and CNKI database 2018 edition of China core journal of science and technology in 9 nursing magazines, That is, Nursing Journal, Journal of Nursing Science, Journal of Chinese Nursing, Journal of PLA Nursing, Journal of Modern Nursing, Journal of Chinese Nursing Management, Journal of Chinese Practical Nursing, Journal of Nursing Research, Journal of Chinese Modern Nursing. Application CiteSpace Ⅴ software to the author, research institutions and key words, such as visual analysis, carried on in-depth analysis to dash forward show at the same time. Results A total of 925 papers were included, with the most published in 2014. There were four main author cooperation groups including Dai Limin, Zhao Xiujun, Yuanli and Wangfang, and only one institution had an obvious cooperative relationship. Hot key words mainly include health education, specialized nursing, diet nursing, etc. The research focuses on diabetes health education, diabetic foot nursing, diabetes diet nursing, self-management, information nursing 5 aspects. In the future, continuous care for diabetes and specialized care for diabetes are increasingly concerned. Conclusion Although diabetes care started late in China, which has developed rapidly and achieved fruitful research results. In the future, more cooperation between institutions should be strengthened, which is conducive to clinical practice and academic development in the field of diabetes care. At the same time, more attention should be paid to the continuation of diabetes care and specialized care.

Effect of 5S management method on nurse′s practical operation ability in the personal nursing terminal information system

Objective To explore the effect of 5S management on the practical operation ability of nurses′ personal care terminal information system. Methods From January 2017 to December 2018, the First Affiliated Hospital of Hebei Northern University selected a total of 178 nurses in 4 departments equipped with personal care terminal information system (PDA), they were divided into experimental group and control group by random number table method, with 89 cases in each group. The experimental group was standardized and guided by 5S management method, while the control group was guided by general management method. The drug administration error rate and work quality of nurses in the two groups were evaluated. Two hundred inpatients in the same time period were included, including 100 patients (5S management group) and 100 patients (general management group) who received 5S management intervention. The satisfaction of nursing service and the quality of nursing care were evaluated. Results The error rate of nurses in the experimental group was lower than that in the control group, and the difference was statistically significant (P < 0.05). The scores of problem solving ability, sense of responsibility, practical operation ability and work efficiency of nurses in the experimental group were all higher than those in the control group, with statistically significant differences (P < 0.05). The satisfaction of patients in 5S management group was (97.00%) higher than that in general management group (79.00%), with statistically significant difference (P < 0.05). Conclusion The 5S management method can improve the practical operation ability of nurses′ PDA, reduce the error rate of nurses′ medication administration, and improve patient satisfaction.

Effect of extreme hepatectomy on quality of life, psychological status and social function of patients with hepatocellular carcinoma

Objective To study the effect of extreme hepatectomy on quality of life in patients with liver cancer. Methods A total of 100 patients with liver cancer who underwent hepatectomy in the First Affiliated Hospital of Hainan Medical College in Hainan Province from January 2015 to January 2018 were selected as study objects, according to the random number table method, they were divided into group A and group B, with 50 cases in each group. Group A was given extreme liver resection, group B was given subtotal hepatectomy. Postoperative quality of life and influencing factors were compared between the two groups. Results There was no statistically significant difference in the incidence of complications between the two groups (P > 0.05). The length of hospitalization in group A was longer than that in group B, and the hospitalization cost was higher than that in group B, the difference was statistically significant (P < 0.05). The number of recurrence cases, number of death cases, body mass index, fitness score, Hamilton anxiety scale score, Hamilton depression scale score, Wechsler test, adaptability test and SF-36 test in group A were all higher than those in group B, with statistically significant differences (all P < 0.05). Conclusion Extreme hepatectomy can optimize the quality of life of patients by reducing the recurrence rate and improving the rehabilitation effect of patients, but the high cost of surgery can become one of the factors affecting the quality of life of patients.

Study on the effect of homology of medicine and food generation of tea on the adjuvant therapy of hypertension

Objective To investigate the effect of homology of medicine and food generation tea on the adjuvant treatment of hypertension and its safety. Methods From October 2018 to June 2019, 75 patients with hypertension were recruited in suzhou Bailian Health Service Center. According to the random number table method, they were divided into the intervention group (38 cases) and the control group (37 cases). The control group received routine treatment and health education, and on the basis of the control group, the intervention group carried out homology of medicine and food tea substitute intervention, the intervention period was 12 weeks. Blood pressure, blood biochemistry and quality of life were compared between the two groups during the intervention. Results After 6 and 12 weeks of intervention, systolic blood pressure in the intervention group was lower than that in the control group (P < 0.05). In addition, there were statistically significant differences in intergroup effect, time effect and interaction between the two groups (all P < 0.05). Intra-group comparison showed that after 6 weeks of intervention, systolic blood pressure in the intervention group was lower than before intervention (P < 0.05). After 12 weeks of intervention, systolic blood pressure in the intervention group was lower than before and 6 weeks after intervention (P < 0.05). The comparison of blood biochemical indicators between the two groups showed that total cholesterol, alanine transaminase, total bilirubin and albumin in the intervention group were lower than before the intervention, with statistically significant differences (P < 0.05). In addition, total cholesterol, urea nitrogen and albumin in the intervention group were lower than those in the control group, with statistically significant differences (P < 0.05). Intra-group comparison of all dimensions of quality of life between the two groups showed that the score of health status and emotional function dimension in the intervention group was higher than that before intervention, while the score of mental health dimension in the control group was lower than that before intervention, with statistically significant differences (all P < 0.05). The score of health status and mental health dimension in the intervention group was higher than that in the control group, and the difference was statistically significant (P < 0.05). In the intervention group, the compliance with drinking and drinking of herbal and food substitutes for tea was 94.74%, and no serious adverse reactions occurred. Conclusion Herbal and food substitutes can help reduce blood pressure, improve the quality of life, and have higher safety.

Application effect of comprehensive nursing measures in clinical nursing of Dexamethasone treatment of pediatric viral myocarditis

Objective To explore the application effect of comprehensive nursing in clinical nursing of Dexamethasone treatment of pediatric viral myocarditis. Methods From April 2017 to December 2018, 96 cases of outpatients and inpatients in Shenyang Hospital of Traditional Chinese Medicine in Liaoning Province were selected, which met the diagnostic criteria of viral myocarditis. The patients were divided into treatment group and control group by double-blind random number table method, 48 cases in each group. Both groups were given conventional treatment and static point Dexamethasone injection. The control group was given routine nursing measures, the treatment group was given comprehensive nursing measures. Both groups were treated for 14 days, after treatment, clinical efficacy, hospital stay, quality of life, creatine kinase (CK), lactate dehydrogenase (LDH), phosphocreatine kinase isoenzyme (CK-MB) index and complications, and nursing satisfaction of the families of the children were compared. The nursing satisfaction of family members in the treatment group was higher than that in the control group, and the difference was statistically significant (P < 0.05). Conclusion Comprehensive nursing measures in Dexamethasone treatment of pediatric viral myocarditis has a significant effect, can improve the clinical symptoms of children, enhance the quality of life. It is worthy of clinical promotion and application.
