Effect of lncRNA PANDAR on Cisplatin chemosensitivity of cervical cancer cells and Cisplatin-resistant cells

Objective To explore the effect of lncRNA PANDAR on the resistance and sensitivity of Cisplatin in cervical cancer cells. Methods Human cervical cancer Cisplatin-resistant cells named HeLa-DDP were constructed. The parent HeLa and HeLa-DDP cells were stimulated with 1, 2, 4, 8, 16 μg/mL doses of Cisplatin. The recombinant PANDAR vectors or lentiviral plasmid of sh-PANDAR were transfected into HeLa-DDP and HeLa cells. All cells were divided into different groups: control group, vector group, PANDAR group, sh-NC group and sh-PANDAR group. Then, PANDAR expression was detected by qRT-PCR. CCK-8 assay was conducted to determine cell viability. Cell apoptosis was analyzed by flow cytometry. Results HeLa-DDP cells exhibited stronger resistance to Cisplatin relative to parent HeLa cells (P < 0.05). In contrast to parent HeLa cells, PANDAR expression was decreased in HeLa-DDP cells (P < 0.05). Overexpression of PANDAR inhibited cell viability under Cisplatin exposure in HeLa-DDP cells relative to control groups (P < 0.05), concomitant with increases in cell apoptosis (P < 0.05). Whilst, PANDAR cessation enhanced cell viability in HeLa-DDP cells under Cisplatin exposure (P < 0.05). Additionally, PANDAR knockdown increased HeLa cell viability upon Cisplatin conditions (P < 0.05). Conclusion PANDAR may regulate cell resistance to Cisplatin in HeLa-DDP and HeLa cells.

Research on neuroprotective mechanism of EPO in hypoxic environment

Objective To discuss the effect of erythropoietin (EPO) on the proliferation of U251 cells and the levels of BMI1 gene transcription in hypoxia environment of cobalt chloride (CoCl2), and lay the foundation for studying the role of EPO on neuroprotection. Methods The experimental cells were divided into control group (0 μmol/mL CoCl2), CoCl2 group (400 μmol/mL CoCl2) and CoCl2+EPO group (400 μmol/mL CoCl2 and 75 U/mL EPO). The hypoxia model was evaluated by measuring the effect of CoCl2 on cell proliferation by CCK-8 method; the effect of EPO on the proliferation of U251 cells was detected by CCK-8 method; the changes of BMI1 gene transcription levels in CoCl2 group and CoCl2+EPO group was detected by qPCR method. Results The cell proliferation levels in the CoCl2 group at 48 h were lower than those in the control group, and the differences were statistically significant (P < 0.05); the proliferation levels in the CoCl2+EPO group were higher than those in the CoCl2 group, with a statistically significant differences (P < 0.05). The levels of transcription of BMI1 gene in CoCl2+EPO group were higher than those in CoCl2 group, and the differences were statistically significant (P < 0.05). Conclusion This study successfully constructed a hypoxia model of human glial U251 cells with CoCl2. EPO promotes U251 cell proliferation by up-regulating the expression of the BMI1 gene under hypoxic conditions, and EPO has a protective effect on nerve cells.

Effect of matrine on RANKL-induced osteoclast differentiation and the expression of TRAF6, c-fos and NFATc1

Objective To investigate the effect of matrine on RANKL-induced osteoclast differentiation and the possible molecular mechanism. Methods RAW264.7 cells were cultured. According to the different addition of drugs (none, osteoclast inducer RANKL and different doses of matrine), they were divided into blank group, induction group, low-dose matrine group (0.5 mg/mL), medium-dose matrine group (1 mg/mL) and high-dose matrine group (2 mg/mL). The expression of TRAF6, c-fos, NFATc1 mRNA and protein were detected by qPCR and Western blot at the 48th hour after administration. Multinucleated osteoclasts were observed by TRAP on the 7th day. The expression of Calcr and Ctsk mRNA was detected by qPCR on the 9th day. The bone resorption lacunae were observed by toluidine blue staining on the 12th day. Results Compared with induction group, the expression of osteoclasts, bone resorption lacuna, Calcr mRNA and Ctsk mRNA decreased in different dosage matrine group. The expression of TRAF6, c-fos and NFATc1 mRNA in different dosage groups of matrine was significantly lower than that in induction group (P < 0.01), and was dose-dependent. There was no significant difference in the expression of TRAF6, c-fos and NFATc1 mRNA between blank group and high dose group of matrine (P > 0.05). Compared with induction group, the expression of TRAF6, c-fos and NFATc1 protein in matrine low, middle and high dose groups decreased (P < 0.05), and it was dose-dependent. There was no significant difference in the expression of TRAF6, c-fos and NFATc1 between blank group and high dose group of matrine (P > 0.05). Conclusion Matrine may affect RANKL-induced osteoclasts differentiation by inhibiting the expression of TRAF6, c-fos and NFATc1.

Expression and clinical significance of DNMT1 and DNMT3a in colorctal cancer and its correlation study

Objective To explore the expression, clinical significance and correlation of DNA methyltransferase-1 (DNMT1) and DNA methyltransferase-3a (DNMT3a) in human colorectal adenocarcinoma tissues. Methods Fifty specimens of colorectal adenocarcinoma tissues (cancer group) and 50 specimens of normal colorectal tissues (normal group) in the First Affiliated Hospital of Hebei North University from February 2017 to February 2018 were selected. The expression levels of DNMT1 mRNA and DNMT3a mRNA were detected by reverse transcription quantitative real-time polymerase chain reaction (qRT-PCR), and the expression levels of DNMT1 and DNMT3a protein were detected by immunohistochemistry technique. The clinical data was collected to analyze the relationship between their expression and clinicopathological characteristics. The correlation was analyzed by Spearman test. Results The qRT-PCR results showed that the expression level of DNMT1 mRNA and DNMT3a mRNA in the cancer group were higher than those in the normal group, the differences were all statistically significant (all P < 0.05). The immunohistochemical staining results showed that the positive rates of DNMT1 and DNMT3a protein in cancer group were higher than those in the normal group, the differences were statistically significant (all P < 0.05). The expression of DNMT1 and DNMT3a protein in cancer group was correlated with the depth of tumor invasion, TNM stage, lymph node metastasis, and liver metastasis (all P < 0.05), which was not correlated with tumor size and degree of differentiation (all P > 0.05). Spearmen correlation analysis showed that there was significant positive correlation between DNMT1 and DNMT3a in cancer tissues (r = 0.455, P < 0.05). Conclusion The abnormal expression of DNMT1 and DNMT3a plays an important role in the occurrence and development of colorectal cancer, they have a synergistic effect, both can be used as potential biological indicators for early diagnosis and treatment monitoring of colorectal cancer.

Network pharmacological research of Qishen Yiqi Dropping Pills in the treatment of heart failure with preserved ejection fraction

Objective To explore the potential active components and mechanisms of Qishen Yiqi Dropping Pills on the treatment of heart failure with preserved ejection fraction (HFpEF) through network pharmacology-based techniques. Methods The active components and target information of Astragalus propinquus Schischkin, Salvia miltiorrhiza Bunge, Ranax Notoginseng(Burk.)F. H. Chen, and Dalbergiae Odoriferae T. Chen were collected by BATMAN-TCM platform. Genecards database and PALM-IST platform was used to collect target genes related to HFpEF. Search date was March 4, 2019. Drug-active components-target network was constructed and visualized by Cytoscape 3.2.1. Topological characteristic of the nodes in the network was analyzed using network analyzer. Target genes interaction was analyzed by STRING database while gene ontology (GO) and pathways enrichment were analyzed by Metascape database. Results A total of 95 potential active components and 262 potential target genes of HFpEF were obtained from Qishen Yiqi Dropping Pills. Gene enrichment analysis showed that Qishen Yiqi Dropping Pills played a role in treatment of HFpEF by regulating biological process including inflammatory response, oxidative stress, myocardial fibrosis and apoptosis. Conclusion This study preliminarily discussed potential active components and mechanism of Qishen Yiqi Dropping Pills on the treatment of HFpEF on a network pharmacological basis, revealing the multi-component, multi-target and multi-channel integrated regulating effect, and provids theoretical information for further experimental research.

Simultaneous determination of two constituents in Chuangui Bushen Tongluo Capsules by high performance liquid chromatography

Objective To establish a high performance liquid chromatography (HPLC) method for simultaneous determination of salvianolic acid B and tanshinone ⅡA in Chuangui Bushen Tongluo Capsules. Methods The content of salvianolic acid B and tanshinone ⅡA was determined by HPLC. Agilent ZORBAX SB-C18 (5 μm, 4.6 mm×250 mm) was used as the column. Acetonitrile(A)-0.1% aqueous phosphoric acid (B) was used as the mobile phase with gradient elution. With the detection wavelength of 270 nm tanshinone ⅡA and 286 nm vianolic acid B. The flow rate was 1.0 mL/min and column temperature was 20℃. Results The linear ranges of salvianolic acid B and tanshinone ⅡA were 44.83-448.3 μg/mL, 0.96-9.60 μg/mL, respectively. The regression equation were Y = 12 650X-1973.3(r = 0.9996),Y = 54 274X+22 206 (r = 0.997), respectively. The average recoveries were 99.7% (RSD = 1.9%), 97.4% (RSD = 1.7%), respectively. Conclusion The established method has the advantages of simple operation, strong specificity and good reproducibility, which can be used to control the quality of Chuangui Bushen Tongluo Capsules.

Simultaneous determination of four constituents in Tangzu San by high performance liquid chromatography

Objective To establish a high performance liquid chromatography method for the simultaneous content determination of four constituents in Tangzu San. Methods SilverSil C18 (4.6 mm×250 mm, 5 μm) column was used as the column, acetonitrile-0.1% phosphoric acid was used as the mobile phase of the gradient elute, and the flow rate of 1.0 mL/min, the colomn temperature maintained at 35℃. With the detection wavelength of 278 nm. Results The regression equations of hydroxy safflower yellow A, paeoniflorin, ferulic acid and β-ecdysterone were Y = 12 183X-5645.7, Y = 10 438X-22 481, Y = 34 706X-1790.1, Y=13 500X-898.66 respectively. The method showed good linearity (all r ≥ 0.9995) within the linear range. The average recoveries were in the range of 96.86%-101.72%. Conclusion The method can provide scientific basis for the quality control of Tangzu San.

Simultaneous determination of Prim-O-Glucnylcimifugin and 4′-O-β-glucosyl-5-O-Methylvismmiside in Compound Jingfang Granules by SPE-UPLC

Objective To establish a solid phase extraction and ultra performance liquid chromatography(SPE-UPLC) method for the simultaneous determination of Prim-O-Glucnylcimifugin and 4′-O-β-glucosyl-5-O-Methylvismmiside in Compound Jingfang Granules. Methods Samples were processed by the solid phase extraction system, and the separation was performed on ACQUITY UPLC HSS T3 (2.1 mm×100 mm,1.8 μm) with acetonitrile-water mobile phase for the gradient elution. The flow rate was 0.4 mL/min, the detection wavelength was 254nm, and the column temperature was 35℃. Results The calibration curve for Prim-O-Glucnylcimifugin and 4′-O-β-glucosyl-5-O-Methylvismmiside in the range of 5-80 mg/L showed good liner. The linear regression equation for Prim-O-Glucnylcimifugin were A = 40 195C-248.67 (r =1.0000) and the linear regression equation for 4′-O-β-glucosyl-5-O-Methylvismmiside were A = 42 577C+12 782(r =1.0000). The average recovery of Prim-O-Glucnylcimifugin and 4′-O-β-glucosyl-5-O-Methylvismmiside were 98.11% and 99.28%, and RSD were 1.92% and 1.90%(n = 6), respectively. Conclusion SPE-UPLC method is simple, accurate and reproducible and effectivly control the quality of Compound Jingfang Granules.

Research progresses in resting-state functional magnetic resonance in primary insomnia

The pathogenesis of insomnia is still unclear. Resting-state functional magnetic resonance (rs-fMRI) provides a powerful technical support for further exploring the changes of brain function in insomnia. In this paper, rs-fMRI studies on primary insomnia in recent years were reviewed and summarized. The results found that abnormal brain areas were mainly located in cognitive-emotional related brain areas such as insula, prefrontal lobe, temporal lobe and limbic system, and there were abnormal internal activities and connections of default network and prominence network. Insomnia is closely related to negative emotions and leads to cognitive impairment and hyperarousal.

Research progress in the treatment of gastrointestinal stromal tumors

Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal tumors of the digestive tract. Surgery is the first choice of treatment for patients with localized or potentially resectable GISTs, but patients still have a high recurrence rate after receiving surgical treatment. The emergence of targeted drugs and the progress of surgical methods have made the current treatment of GISTs appear to be minimally invasive, phased, and multidisciplinary, which can better improve the living standards of patients with gastrointestinal stromal tumors. In view of the complexity of the pathogenesis of GISTs and the instability of clinical treatment and prognosis, the treatment of patients with GISTs is now described.

Role of androgen receptor splice variants 7 in castration-resistant prostate cancer and its clinical significance

Castration-resistant prostate cancer (CRPC) is a prostate cancer that progresses after initial sustained endocrine therapy. The mechanism of CRPC occurrence and development is extremely complicated and has not been fully studied yet, but the continuous activation of androgen receptor (AR) signal is still an important factor. Androgen receptor splice variants 7 (AR-V7) is one of the most important variants of AR. Many studies have shown that it is closely related to the development of CRPC, promoting AR reactivation, tumor growth and metastasis. It plays an important role in the development of drug resistance, and is associated with the sensitivity of endocrine therapy and the overall survival rate. It is a potential evaluation index. In-depth study of the relationship between AR-V7 and CRPC can not only guide the current use of CRPC, it can also provide new ways and directions for the treatment of CRPC.

Advances in blocking NF-κB signaling pathway with traditional Chinese medicine to inhibit herpes simplex virus type Ⅰ infection

Nuclear transcription factor-κB (NF-κB) is an important transcription factor that induces antiviral gene expression and plays an important role in physiological and pathological processes such as oxidative stress, apoptosis and inflammation by regulating the expression of multiple target genes. Herpes simplex virus type Ⅰ (HSV-1) is a widely spread virus, caused a variety of diseases worldwide. The emergence of drug-resistant strains has increased the urgency of developing new antiviral drugs, and medicinal plants and their extracts have attracted more and more attention as antiviral drugs. This article mainly summarizes the mechanism of latent infection and recurrence of HSV-1, the mechanism of NF-κB activation induced by HSV-1 infection and different traditional Chinese medicine blocking the NF-κB signaling pathway inhibition HSV-1 mechanisms, in order to against the new drug research and development and control of HSV-1 virus caused by the disease provide a reference basis.

Research progress of miR-223 and pregnancy-related diseases

MicroRNAs (miRNAs) are a class of endogenous non-coding single-stranded RNA molecules of approximately 21-25 bases that have been found in different species and are involved in a variety of biological processes, such as embryonic development, apoptosis, cell division, hematopoiesis, carcinogenesis and viral infection, etc. MicroRNA 223 (miR-223) is a type of miRNA targeting the q12 chromosome of X chromosome. The study found that miR-223 can affect early embryo transfer by inhibiting the expression of leukemia inhibitory factor, insulin-like growth factor. It is abnormally low expressed in peripheral blood and placenta of preeclampsia, involved in the development of preeclampsia. It is low expressed in cardiomyocytes and overexpression in subcutaneous fat of pregnant women with gestational diabetes, regulating the glucose metabolism in the body through different mechanisms. It is highly expressed in pregnant women with premature rupture of membranes. The biological process of the target gene involved in the negative regulation of immune response and anti-inflammatory response, is considered to be closely related to chorioamnionitis. In addition, miR-223 has high expression in children with Down′s syndrome the amount. This article will summarize the research progress of miR-223 expression level and pathological pregnancy such as embryo implantation, pre-eclampsia and gestational diabetes.

Application progress of triple antibiotic paste in the treatment of periodontal disease and endodontic disease

The oral microecotogical environment is anfractuous, and a variety of pathogenic bacteria will lead to infection of root canal, periodontitis, and periapical periodontitis. The occurrence of these diseases are related to the structure of teeth and periodontal tissues because they will induced different kinds of pathogenic bacteria to accumulate in the corresponding parts. In order to effectively deal with this kind of disease, the application of antibiotic combination is particularly important. As a combination of three kinds of antibiotics, such as Ciprofloxacin, Metronidazole and Minocycline, triple antibiotic paste (TAP) has a good antibacterial effect, which has been used to control the infection in root canals and to promote pulp revascularization. This article mainly reviews and summarizes the application of TAP in the treatment of periodontal disease and endodontic disease.

Application of “presentation-assimilation-discussion class” combined with standardized patients in practice of cardiovascular medicine

Objective To investigate the application effect of “presentation-assimilation-discussion (PAD) class” model combined with standardized patients in practice of cardiovascular medicine. Methods From June to September 2018, 60 cases of clinical interns were selected from Department of Cardiology, Affiliated Hospital of Southwest Medical University and divided into experimental group and control group in accordance with the random number table. There were 30 interns in each group. The experimental group accepted the model of PAD class combined with standardized patients. The control group accepted traditional teaching method. At the end of the internship in cardiovascular medicine, the theoretical examination, medical record writing, operation skills and teaching effect between two groups were evaluated. Results The scores of theoretical knowledge, medical record writing and related skill operation in experimental group were significantly higher than those in control group, the differences were statistically significant (P < 0.05). The evaluation satisfaction of stimulating learning interest, consolidating medical knowledge, improving self-study ability, analyzing problem-solving ability, doctor-patient communication ability, clinical thinking ability, team cooperation ability and literature retrieval ability of experimental group were higher than those of control group, the differences were statistically significant (P < 0.05). Conclusion The teaching model of PAD class combined with standardized patients can significantly improve the teaching effect of practice in cardiovascular medicine, enhance the theoretical knowledge, medical record writing ability as well as operation skills, finally improve the quality of clinical interns.

And four-dimension suggestions “focusing on talent training, clarifying core competence construction; setting up research field in line with national conditions; setting up curriculum system centered on capacity building; and constructing a reasonable educational evaluation mechanism” are put forward to optimize the academic doctor training model of healthcare management in China.

Research on the cultivation path of Chinese medicine talents under the background of comprehensive health

With the advent of the era of comprehensive health, a major revolution in the medical health industry is quietly taking place, facing unprecedented opportunities. At the same time, the comprehensive health industry has put forward more advanced requirements for the cultivation structure of Chinese medical talents. It requires service-oriented, composite, innovative, research-oriented, and practical talents. However, many problems still exist in China′s talent training system resulting in unable to meet social needs, such as the lack of cultural foundation and Chinese classical learning, the insufficient mastery of classic works, the improper combination of clinical research methods and scientific and technological means, the inadequate clinical and practical capacity, the lack of contact between professions, etc. In order to improve the talent training system for colleges and universities, Chinese medicine colleges and universities should follow the direction of Chinese medicine, take career development as the goal, take social demand as the guide, introduce enterprise evaluation into the assessment, let the practice run through the whole process, and establish a new system for training Chinese medicine professionals who serve the public.

Application of curriculum ideological and political education combined with PDCA model in medical course teaching

The curriculm of ideological and political education is an educational and teaching concept that integrates the elements of moral education into each course, so that classroom teaching severs as the main channel for implementing ideological and political education and teaching students with moral education. This paper explores the teaching effect of the curriculum of ideological and political education being applicated through the PDCA model into the course of neurology. With the curriculum of ideological and political practice running through the whole process of the neurology course by applying the PDCA model, the teaching content, teaching methods and evaluation mechanism can be organically combined to make the teaching activities of teachers smooth, well-organized and effective. At the same time, the classroom teaching design can be improved effectively, the students′ feedback on knowledge can be obtained in time, and form a constructive solution that can be copied and promoted. See to it that the curriculum of ideology and political education becomes the conscious behavior of teachers teaching and educating students, as well as realizes that teaching benefits teachers and students in the practical sense.

Effect of effective teaching behavior of clinical nursing teachers on the adaptability of nursing students

Objective To explore the effect of effective teaching behavior of clinical nursing teachers on the adaptability of nursing students. Methods The questionnaire of general information, effective teaching behavior questionnaire of clinical nursing teachers and college students′ career adaptability scale were used to investigate the 278 nursing students in a Grade 3A hospital in Urumqi from October 2017 to March 2019. Results The total score of 278 intern nurses in a Grade 3A hospital in Urumqi was (17.95±5.01) scores, and the total score of nursing students′ career adaptability was (32.95±9.05) scores. The total score of intern nurses′ effective teaching behavior on clinical nursing teachers was positively correlated with the total score and dimensions of intern nurses′ career adaptability (P < 0.05). Univariate analysis showed that different age, sex, education level, clinical practice time, attitude to nursing specialty and evaluation of practice environment had an impact on nursing students′ career adaptability level (P < 0.05). Stratified regression analysis showed that effective teaching behavior of clinical nursing teachers could cause 34.4% variation of nursing students′ career adaptability score. Conclusion The effective teaching behavior of clinical nursing instructors has a certain impact on the career adaptability of nursing students. It can be considered to enhance the career adaptability of nursing students by improving the effective teaching behavior of clinical nursing instructors, thereby improving the quality of nursing.

Application of backward design in the basic teaching of general medicine

Strengthening training for general doctors is now a prominent and demanding requirement to necessarily meet the future health needs of Chinese people. Backward design is a method used to design an educational curriculum, which is a teaching method that ends with learning goals. Based on the characteristics of the medical basics course “Medical Microbiology”, this paper summarizes how the backward design is designed according to the rationale, and operated in five aspects as the studying subjects, the final learning objectives, the methods for teaching, the curricular and pedagogical settings, and the final conclusion and evaluation for all teaching activities, hoping to delve into the possible implementation of backward design in basic teaching of general medicine in medical colleges.

Expression and clinical significance of epidermal growth factor and vascular endothelial growth factor in patients with different phenotypes of bronchial asthma

Objective To investigate the expression and clinical significance of epidermal growth factor (EGF) and vascular endothelial growth factor (VEGF) in patients with different phenotypes of bronchial asthma. Methods A total of 90 patients with acute bronchial asthma admitted to the Second Affiliated Hospital of Xinjiang Medical University from August 2017 to December 2018 were selected as the research objects. The subjects were divided into neutrophil phenotype group (24 cases) and eosinophil phenotype group (66 cases) according to induced type of sputum cell. The serum and sputum levels of EGF and VEGF in the two groups of patients were compared, and the correlation between level of serum eosinophils (EOS) and EGF, VEGF in the eosinophil phenotype group was further analyzed. Results The levels of EGF and VEGF in serum and induced sputum of the patients in the eosinophil phenotype group were significantly higher than those in the neutrophil phenotype group, and the differences were statistically significant (P < 0.05). Serum EOS was positively correlated with serum EGF and VEGF (r = 0.513, 0.397, all P < 0.05), and also positively correlated with the levels of induced sputum EGF and VEGF (r = 0.378, 0.528, all P < 0.05). Conclusion EGF and VEGF in serum and induced sputum of eosinophil phenotype asthma patients are significantly higher than those of neutrophil phenotype bronchial asthma patients, and are correlated with serum EOS levels. The detection of serum EGF and VEGF can simplify the classification and treatment process of bronchial asthma patients, provide help for the treatment of different phenotypes of bronchial asthma.

Effects of baduanjin based on virtual reality on fall risk and quality of life of elderly osteoporosis patients in nursing institution for the aged

Objective To explore the effects of baduanjin based on virtual reality (VR) on the risk of falls and quality of life of elderly patients with osteoporosis (OP) in nursing institution for the aged. Methods From July 2016 to December 2017, 64 elderly OP patients in Nanjing Jiangning Muchunyuan Nursing Home were selected for the study. They were divided into the control group and the observation group according to the random number table method, with 32 cases in each group. Both groups received conventional anti-osteoporosis treatment. On this basis, the observation group adopted baduanjin based on VR training three times a week for a total of 12 months, each time 50 min. Physical function, fall effectiveness, pain level, bone mineral density (BMD), the number of falls and quality of life were observed in both groups of patients. Results During the study, 2 cases fell out in each group. In the end, there were 30 cases each in the control group and the observation group. Before treatment, there was no statistically significant differences in the short physical performance battery (SPPB) score, the modified falls efficacy scale (MFES) score, the visual analogue scale (VAS) score and the short-form 36 item health survey questionnaire (SF-36) score between the two groups of patients (P > 0.05). After 12 months of treatment, SPPB, MFES and SF-36 scores in both groups of patients were significantly higher than those before treatment (P < 0.05), and VAS scores were significantly lower than those before treatment (P < 0.05). SPPB, MFES and SF-36 scores in the observation group were significantly higher than those in the control group (P < 0.05), and VAS scores were significantly lower than those in the control group (P < 0.05). Before treatment, BMD values of lumbar spine (L2-4), femoral neck and total hip of the two groups of patients were not significantly different (P > 0.05). After 12 months of treatment, BMD values in both groups of patients were significantly higher than those before treatment (P < 0.05), and BMD values in the observation group were significantly higher than those in the control group. Within 12 months before treatment, there was no significant difference in the number of falls between the two groups of patients (P > 0.05). Within 12 months after treatment, the number of falls of patients in both groups was significantly lower than that before treatment (P < 0.05), and the number of falls in the observation group was significantly lower than that in the control group (P < 0.05). Conclusion Based on VR, baduanjin can effectively improve the physical function, fall efficiency, lower back pain and BMD of elderly OP patients in nursing institution for the aged, reduce the risk and number of falls, improve their quality of life, and facilitate the promotion and application in nursing institution for the aged.

Changes and clinical significance of intercellular adhesion molecule-1 and eosinophils levels in patients with bronchial asthma

Objective To investigate the changes and clinical significance of intercellular adhesion molecule-1 (ICAM-1) and eosinophils (EOS) in patients with bronchial asthma. Methods From January 2012 to December 2017, a total of 86 patients with bronchial asthma treated in Nanjing Brain Hospital (“our hospital” for short) were divided into acute attack group (45 cases) and remission group (41 cases) according to their condition and symptoms. A total of 52 people undergoing health examination in our hospital at the same time were selected as control group. The levels of serum ICAM-1, EOS and pulmonary function were compared among the three groups, and the correlation was analyzed. Results The level of ICAM-1 and EOS in acute attack group and remission group were significantly higher than those in control group, while acute attack group was significantly higher than remission group, and the differences were statistically significant (P < 0.05). Maximal voluntary ventilation (MVV)/predicted value, peak expiratory flow (PEF), one second forced breath volume (FEV1) and forced vital capacity (FVC) in acute attack group and remission group were lower than those in control group, while those in acute attack group were lower than those in remission group, and the differences were statistically significant (P < 0.05). Spearman correlation results showed that ICAM-1, EOS levels were negatively correlated with FVC, FEV1 in patients with bronchial asthma(r = -0.652, -0.548; r = -0.549, -0.598, P < 0.05). Conclusion ICAM-1 and EOS may be involved in the occurrence and development of bronchial asthma. The analysis of ICAM-1 content and EOS count have important clinical significance for the diagnosis and treatment of bronchial asthma.

The relationship between serum adiponectin, cystatin C and glucolipid metabolism in diabetic limb artery occlusion

Objective To investigate the correlation between serum adiponectin (APN) and cystatin C (CysC) on glucolipid metabolism in diabetic limb artery occlusion (DLAO). Methods From July 2017 to December 2018, 77 patients with the first stage or the second stage DLAO and 76 patients with diabetes mellitus (DM) admitted to Shanghai TCM-Integrated Hospital, Shanghai University of Traditional Chinese Medicine were selected as the study objects, named as DLAO group and DM group respectively. Body mass index (BMI), waist hip ratio (WHR) and serum total cholesterol (TC), triacylglycerol (TG), high-density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), apolipoprotein A1 (Apo-A1), apolipoprotein B (Apo-B), fasting blood glucose (FBG), glycosylated hemoglobin (HbA1c), CysC, APN levels were measured for all subjects. Logistic stepwise regression analysis with WHR, APN, CysC and Apo-A1 as independent variables was conducted for DLAO patients. Pearson correlation analysis was conducted between serum APN and BMI, WHR, course of diabetes, TC, TG, HbA1c, CysC. Results The levels of WHR and serum CysC in the DLAO group were significantly higher than those in the DM group. Serum APN and Apo-A1 in the DLAO group were significantly lower than those in the DM group, with statistically significant differences (all P < 0.05). FBG, HbA1c, BMI, TC, TG, HDL-C, LDL-C and Apo-B of parents in the two groups showed no statistically significant differences (all P > 0.05). Logistic stepwise regression analysis found that high serum APN was the independence protective factor for DLAO, while high serum CysC was the independence risk factor for DLAO (P < 0.05). Pearson correlation analysis showed that serum APN level was negatively correlated with WHR, diabetes course, and serum CysC level (r < 0, P < 0.05), but not with BMI, TC, TG, and HbA1c (P > 0.05). Conclusion Serum APN and CysC have a certain effect on glucolipid metabolism during the formation and development of DLAO.

Diagnostic value of serum Hcy, ET-1 and urine U-mAlb/Cr ratio in early renal injury patients with preeclampsia

Objective To explore the diagnostic value of serum homocysteine (Hcy), endothelin-1 (ET-1) and urinary microalbumin/creatinine (U-mAlb/Cr) in early renal injury patients with preeclampsia. Methods From March 2018 to May 2019, 95 cases of preeclampsia patients admitted to Tangshan Maternal and Child Health Hospital of Hebei Province were studied. According to the results of glomerular filtration rate, they were divided into abnormal renal function group (42 cases) and normal renal function group (53 cases). Another 60 healthy pregnant women were selected as control group. Serum levels of Hcy, ET-1, U-mAlb and urine Cr levels were detected. Results Compared with the control group, the levels of serum Hcy, ET-1 and urinary U-mAlb/Cr in the abnormal renal function group and the normal renal function group increased (P < 0.05). Compared with the normal renal function group, the levels of Hcy, ET-1 and U-mAlb/Cr in urine of the abnormal renal function group were increased (P < 0.05). Glomerular filtration rate, Hcy, ET-1 and U-mAlb/Cr were independent risk factors for early renal injury in preeclampsia patients. The AUC of Hcy, ET-1, U-mAlb/Cr and three combined diagnostic methods for ROC in early renal injury was 0.815, 0.774, 0.832 and 0.892, respectively, the sensitivity was 75.78%, 73.68%, 78.94%, 88.42%, and the specificity was 78.94%, 75.78%, 85.26% and 81.05%, respectively. Conclusion The levels of serum Hcy, ET-1 and U-mAlb/Cr increase after early renal injury in preeclampsia patients, and they have certain diagnostic value.

Changes of hs-CRP,AAG and HP level and its clinical significance in children with pneumonia

Objective To discuss the clinical significance of hypersensitive acute-phase protein (hs-CRP), α1-acid glycoprotein (AAG), haptoglobin (HP) and ceruloplasmin (CP) levels in the surveillance of children with pneumonia. Methods From January to June 2017, 96 children with pediatric pneumonia who were treated in the Second People′s Hospital of Hengshui in Hebei Province (“our hospital” for short), and they were divided into three groups according to the etiology, including 49 cases in bacterial pneumonia group, 27 cases in Mycoplasma pneumonia group, 20 cases in viral pneumonia group. Another 60 healthy children examined in our hospital during the same period were selected as the healthy control group. The hypersensitive C-reactive protein (hs-CRP), alpha-1 acid glycoprotein (AAG), haptoglobin (HP) and ceruloplasmin (CP) of each groups were tested and compared. Results The levels of hs-CRP, AAG and HP in bacterial pneumonia group and Mycoplasma pneumonia group were significantly higher than those in the viral pneumonia group and healthy control group, with statistically significant differences (P < 0.05). The levels of hs-CRP, AAG and HP of children with severe pneumonia from bacterial pneumonia group and Mycoplasma pneumonia group were higher than those of children with mild pneumonia. The levels of hs-CRP, AAG and HP increased with severity of disease, with statistical significance (P < 0.05). The levels of AAG and HP were significant positively correlated with hs-CRP (r = 0.7074, 0.7874, P < 0.01). The level of AAG was significantly positively correlated with HP (r = 0.6882, P < 0.01). There was no significant correlation between the level of CP hs-CRP, AAG and HP (P > 0.05). Conclusion The levels of hs-CRP, AAG and HP in bacterial pneumonia and Mycoplasma pneumonia children were significantly higher than those in the viral pneumonia children and the healthy children. There appeared some well correlation between the levels of hs-CRP, AAG and HP. The three indexes are helpful to a certain extent to identify childhood pneumonia and evaluate the severity caused by pneumonia in children. This provides significance for the clinical treatment of pneumonia in children.

Survival benefit analysis of high flow oxygen therapy and standard oxygen therapy for patients with low immune function AHRF

Objective To investigate the survival benefit between high flow oxygen therapy and standard flow oxygen therapy for patients with low immune function AHRF. Methods From May 2017 to March 2019, a total of 72 cases in 3 intensive care units of Shanghai General Hospital and Shanghai Oriental Hospital were selected. According to the month of admision, the parity distribution method was adopted, they were divided into the high flow group and the standard flow group, with 36 cases in each group. The high-flow group was provided by oxygen therapy device and the standard flow group was provided by any device or combination of devices. All patients were observed for 28 d. The total mortality of 28 d, the number of deaths in and out of the ICU, the number of invasive ventilation, the number of acquired infections in the ICU, the length of stay in the ICU and the total length of stay in hospital were compared between the two groups. The risk ratio of randomized factor to mortality and cumulative invasive mechanical ventilation rate on 28 d were analyzed. Post-hoc analysis used a model and subset analysis of intubated patients. Results There were no statistically significant differences in terms of total death of 28 d, in and out of ICU death, invasive ventilation, and total hospital stay between the two groups (P > 0.05). The ICU acquired infection in the high flow group was lower than that in the standard flow group, and the length of stay in the ICU was shorter than that in the standard flow group, with statistically significant differences (P < 0.05). Randomization factors had no statistical significance on the mortality rate of 28 d (P > 0.05), and had a statistically significant effect on the cumulative invasive mechanical ventilation rate (P < 0.05). Conclusion Compared with standard flow oxygen therapy, high flow oxygen therapy does not significantly reduce the mortality rate and cumulative invasive mechanical ventilation rate in 28 d, but can improve the oxygenation index (PaO2/FIO2) in a short time, shorten the hospitalization time in ICU and reduce the rate of acquired infection in ICU.

Clinical effect of insomnia treatment with the method of acupoint application and four-color personality analysis

Objective To observe the clinical effect of insomnia treatment with the method of acupoint application of Chinese herb and four-color personality analysis. Methods From January 2016 to January 2017, 90 cases with insomniacs treated in Hangzhou Hospital of Traditional Chinese Medical were selected as the objects. According to the random number table method, they were divided into the basic group, the application group and the combined group, with 30 cases in each group. The basic group received routine care, the application group were given acupoint application on bilateral yongquan point on the basis of routine treatment, the combined group applied four-color personality analysis to psychological nursing on the basis of the application group. The clinical effect, scores of Pittsburgh sleep quality index (PSQI) and the rate of adverse reaction were compared among the three groups. Results The total effective rate of the combination group was higher than that of the application group and the basic group, and the total effective rate of the application group was higher than that of the basic group, with statistically significant differences (all P < 0.05). There was no significant difference in PQSI scores before nursing in the three groups (P > 0.05). After nursing, the PQSI scores of the three groups were lower than those before nursing, and the combination group was lower than the application group and the basic group, the application group was lower than the basic group, with statistically significant differences (all P < 0.05). And there was no significant difference in the occurrence of adverse reactions among the three groups (P < 0.05). Conclusion The clinical effect of insomnia treatment with the method of acupoint application and four-color personality analysis was significant. It can effectively improve patients′ anxiety and sleep condition by prolonging their sleep time, which is safe, reliable and worthy of promotion.

Application of ultrasound-guided double-line localization in radial artery puncture

Objective To investigate the clinical effect of ultrasound-guided double-line localization in radial artery puncture. Methods From September 2018 to June 2019, a total of 80 patients who planned to undergo radial artery puncture in Beijing You′an Hospital of Capital Medical University were selected. According to the random number table method, they were divided into control group and observation group, with 40 cases in each group. Control group received conventional ultrasound-guided radial artery puncture, and observation group received double-line localization technology. The success rate of the first puncture, the rate of puncture failure, ultrasonic positioning time and the incidence of complications were compared between two groups. Results The success rate of the first puncture in observation group was higher than those in control group, the ultrasonic positioning time of observation group was shorter than that of control group, and the differences were statistically significance (all P < 0.05). There was no significant difference in the rate of puncture failure between two groups (P > 0.05). The incidence of bleeding in observation group was lower than that in control group, and the difference was statistically significant (P < 0.05). Conclusion The first puncture success rate and ultrasonic positioning time of the radial artery puncture under the guidance of ultrasound can be improved significantly under the double-line localization technique.

Effect of Zhuangfang Ruogan Huaxian Granules in the treatment of ascites due to hepatitis B cirrhosis with syndrome of liver-kidney yin deficiency

Objective To observe effect of Zhuangfang Ruogan Huaxian Granules in the treatment of ascites due to hepatitis B cirrhosis with syndrome of liver-kidney yin deficiency. Methods From February 2017 to February 2019, 70 patients with hepatitis B cirrhosis ascites with syndrome of liver-kidney yin deficiency were selected from the First Affiliated Hospital of Guangxi University of Chinese Medicine and Hepatology Department of Guangxi International Zhuang Medical Hospital. They were divided into control group and observation group according to the random number table method, with 35 cases in each group. Control group received routine medical treatment, while observation group received Zhuangfang Ruogan Huaxian Granule on the basis of control group. Total effective rate after treatment, traditional Chinese medicine syndrome score, liver function index, body mass, abdominal circumference and 24 h urine volume before and after treatment were compared between two groups. Results Total clinical effective rate of observation group was higher than that of control group (P < 0.05). The scores of jaundice, anorexia, abdominal distention, diarrhea, hepatosplenomegaly in two groups after treatment were lower than those before treatment, observation group was lower than control group, and the differences were statistically significant (P < 0.05). The levels of AST, ALT and TBIL in two groups after treatment were lower than those before treatment, and the level of ALB in observation group after treatment was higher than that before treatment, and the improvement in observation group was more significant than that in control group, and the differences were statistically significant (P < 0.05). Compared with those before treatment, body mass in the two groups decreased, abdominal circumference reduced, and 24 h urine volume increased after treatment, and the improvement in observation group was more significant than that in control group, and the differences were statistically significant (P < 0.05). Conclusion Zhuangfang Ruogan Huaxian Granules can obviously improve the therapeutic effect and improve the liver function and quality of life of patients with ascites due to hepatitis B cirrhosis with syndrome of liver-kidney yin deficiency.

Discussion on Professor Duan Guangtang′s experience in treating chronic renal failure with the method of invigorating the spleen and kidney, excreting turbid and removing blood stasis

Professor Duan Guangtang considers that the basic pathogenesis of chronic renal failure is deficiency in origin and enrichment in symptom. The former is asthenia of the spleen and kidney, the latter is accumulated blood stasis and damp turbidity. The basic rules for the treatment of this disease are invigorating the spleen and kidney, excreting turbid and removing blood stasis. The article introduces Professor Duan Guangtang′s academic viewpoint and clinical experience from the aspects of the pathogenesis and treatment of pruritic dermatosis, the presciption analysis of the NO.1 and NO.4 Decoction, and the clinical application of this method in the treatment of chronic renal failure diseases. Took the chronic kidney failure caused by glomerular disease, the main clinical manifestations included spleen-kidney qi(yang) deficiency; while the chronic kidney failure caused by renal tubular interstitial disease, the main manifestations included kidney-qi(yang) deficiency. Mainly tonifying the spleen and the kidney is adopted, we achieve well curative effect by using NO.1 and NO.4 Decoction to treat this disease, the therapy of Chinese medicine can curb the deterioration progress of renal function for thousand patients, thereby delay the time before entering dialysis therapy. Polycystic ovary syndrome (PCOS) belongs to the category of “late menstruation”“amenorrhea”“infertility” in traditional Chinese medicine. It is one of the common diseases that cause endocrine metabolism disorder and affect fertility of women of childbearing age. Over the years, there have been many traditional Chinese medicine theories about PCOS infertility, and there are many acupuncture treatments for PCOS. This paper analyzes the clinical diagnosis and treatment of PCOS according to the etiology and pathogenesis of PCOS in traditional Chinese medicine, the circulation of Conception-Governor Channel and its main therapeutic effect. On this basis, it puts forward that the theory of connecting and regulating should be used to guide acupuncture and moxibustion in order to be an effective way to prevent and treat the non pregnancy of PCOS.

Based on the analysis of relevant ancient literatures, this paper mainly summarizes the mechanism of dampness in the pathogenesis of pediatric asthma, and the ancient and modern treatment characteristics for asthma based on dampness. Exogenous dampness evil is one of the important causes of pediatric asthma. Based on the physiological characteristics of lung, spleen and kidney insufficiency in children, exogenous dampness evil is easy to cause phlegm turbidity, qi deficiency, yin deficiency, phlegm-stasis, and so on. Pathological products such as phlegm-stasis can further inhibition qi activity and the process of body fluid metabolism, which will aggravating the clinical seizures of asthma. According the nature of dampness and physiological characteristics of children, we commonly used the medicine with curative effective of resolving phlegm, relieving cough, removing dampness, invigorating spleen, benefiting lung, and tonifying kidney, which can achieve treating both root causes and symptoms.

Clinical effect of eliminating pathogens from lung for expelling fluid retention method in the treatment of tuberculous pleurisy

Objective To explore clinical effect of eliminating pathogens from lung for expelling fluid retention method in the treatment of tuberculous pleurisy. Methods From February 2017 to January 2019, 86 patients with tuberculous pleurisy were randomly divided into control group and treatment group according to coin tossing method, with 43 cases in each group. After being admitted to hospital, control group was given routine Western medicine anti tuberculosis treatment. Treatment group was treated on the basis of Western medicine combined with eliminating pathogens from lung for expelling fluid retention method. Both groups were treated for 2 months continuously. Clinical effect after treatment, traditional Chinese medicine syndrome score, tumor necrosis factor-α (TNF-α) and C-reactive protein (CRP) levels in pleural effusion before and after treatment were compared between two groups. Results Total effective rate of treatment group was higher than that of control group after treatment (P < 0.05). After treatment, scores of traditional Chinese medicine syndromes in two groups were lower than those before treatment, treatment group was lower than control group, the difference was statistically significant (P < 0.05). After treatment, levels of TNF-α and CRP in two groups were lower than those before treatment, treatment group was lower than control group, and the difference was statistically significant (P < 0.05). Conclusion Eliminating pathogens from lung for expelling fluid retention method combined with Western medicine anti tuberculosis treatment can improve the clinical effect of tuberculous pleurisy patients, better improve the symptoms of patients, reduce the inflammatory level of patients, the clinical effect is significant, it is worthy of clinical promotion.

Professor Qiu Qingzhong applies rattan drugs to different stages of arthralgia by analyzing their characteristics and compatibility. Professor Qiu believes that the early stage of arthralgia is mainly caused by injury, cold coagulation, qi stagnation and blood stasis, which result in qi stagnation, blood stasis, phlegm-dampness and other pathological products. In the later stage, both liver and kidney are deficient. Long time no cure, the heart, liver and spleen can be involved. Rattan drugs should be used throughout the treatment of arthralgia. They are good at the meridians and collaterals of the table, and compatible with insect drugs can search for wind and dredge collaterals; supplemented by drugs such as promoting qi, removing blood stasis and removing dampness, they can treat both the root causes and symptoms; compatible products of tonifying liver and kidney can be used simultaneously; supplemented by traditional Chinese medicines such as nourishing heart, soothing liver and invigorating spleen can regulate mood, invigorate the spleen and calm the mind; and direct access to the sick place can be achieved by the use of meridian-guiding herb.

Study on the similarities and differences of the effectiveness criteria of acupuncture in cervical spondylosis

Objective To find out the commonly used standards/scales for judging the curative effect of acupuncture on cervical spondylosis in China, and sort out the contents concerned by different standards/scales, so as to find out the common contents for judging the curative effect, and lay the foundation for further determining the core index set of acupuncture on cervical spondylosis. Methods By using the method of literature research, it was searched and published in CNKI and Wanfang database from January 2015 to January 2019, with the subject of “cervical spondylosis” combined with “efficacy standard” and the key word of “acupuncture”. Finally, 60 articles were included in the literature research, and the literature metrological analysis was carried out around the criteria and contents (dimensions, items), the main outcome and its measurement tools, scoring methods, secondary outcome and its measurement tools, scoring methods, etc. of acupuncture treatment for cervical spondylosis. Results The effect of acupuncture in the treatment of cervical spondylosis was often judged by The standard of diagnosis and curative effect of diseases and syndromes of traditional Chinese medicine. The main outcome of acupuncture treatment of cervical spondylosis was efficiency, while the secondary outcome was pain. Pain and sensory disturbance in the commonly used standard/scale of acupuncture treatment of cervical spondylosis were the dimensions of more concern. The scores of neck pain, shoulder back pain and other items accounted for a large proportion. Conclusion The effect evaluation of acupuncture therapy for cervical spondylosis in China should pay attention to the patients quality of life. The effectiveness focus on the effective rate and pain. The standard/scale dimension and item content are different. To develop the specific scale of acupuncture treatment for cervical spondylosis, or to establish the core index set of acupuncture treatment for cervical spondylosis.

Application value of dynamic contrast-enhanced MRI combined with diffusion-weighted magnetic resonance imaging in staging diagnosis of cervical cancer

Objective To explore the application effect of DCE-MRI combined with DWI (DCE-MRI+DWI) in staging diagnosis of cervical cancer and to guide clinical diagnosis. Methods From December 2017 to December 2018, 83 patients with cervical cancer confirmed by pathological examination in Huangjiahu Hospital, Hubei University of Chinese Medicine were selected. All patients underwent routine MRI and dynamic contrast-enhanced MRI combined with DWI before operation, the accuracy of the two methods of inspection were explored. Results There were significant differences in Ktrans and kep values of DCE-MRI and ADC value of DWI in patients with different FIGO clinical stages and differentiated degrees of cervical cancer (P < 0.05). The accuracy of diagnosis of cervical cancer pathological staging with conventional MRI and DCE-MRI+DWI was 62.65% and 95.18%, respectively, and the difference was statistically significant (P < 0.05). In the analysis of tissue invasion of adjacent tissues, the sensitivity and specificity of conventional MRI were 83.33% and 78.72%, respectively. The sensitivity and specificity of DCE-MRI+DWI were 97.22% and 95.74%, respectively. The sensitivity and specificity of DCE-MRI+DWI were higher than the conventional MRI, and the differences were statistically significant (P < 0.05). Conclusion Compared with conventional MRI, DCE-MRI+DWI is more accurate in the diagnosis of cervical cancer staging, which is worthy of clinical application.

Correlation between CT diagnosis and BRAF gene in benign and malignant thyroid nodules

Objective To explore the correlation between CT diagnostic characteristics and BRAF gene in benign and malignant thyroid nodules. Methods A total of 87 patients with pathologic and CT data were collected from January 2015 to December 2017 in Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine. CT features of benign and malignant nodules were recorded and univariate analysis was performed. The mutation of BRAF V600E gene was detected simultaneously. Results In 87 cases of thyroid nodules, 103 nodules were found, including 62 benign nodules and 41 malignant nodules. The CT features of benign group were nodular morphology, clear edges, and cystic degeneration can be seen in thyroid nodules. Benign nodules showed uneven enhancement and clear relationship with surrounding tissues after enhanced scanning. The CT features of malignant group were blurred nodules with irregular shape and granular calcification. Enhanced scan of malignant nodules showed enlargement of the volume of the lesion, some of them showed “peritumoral enhancement residual circle” or “peninsular enhancement”, and enlarged lymph nodes. Compared with benign nodules, malignant nodules had significant differences in number, shape, border, enhancement and enlarged lymph nodules (P < 0.05). The mutation rate of exon 15 of BRAF gene V600E in malignant nodules was significantly higher than that in benign nodules (P < 0.05). Conclusion The CT feature of thyroid nodule has certain characteristics. Combined with the mutation detection of BRAF V600E gene, CT images have some reference value for reducing missed diagnosis and misdiagnosis.

Analysis of imaging features of unsuspected gallbladder cancer and causes of misdiagnosis or missed diagnosis

Objective To summarize imaging fearures of unsuspected gallbladder cancer (UGC), analyze causes of missed diagnosis or misdiagnosis and provide prevention method, in order to reduce the incidence of UGC. Methods The preoperative CT and MRI imagings of 12 UGC patients who treated in the Affiliated Drum Tower Hospital of Nanjing Medical University from January 2016 to December 2018 were retrospectively analyzed and compared with the pathological results. Results The postoperative pathology diagnosis of the 12 cases were confirmed as gallbladder cancer. There were 8 cases out of 12 cases were misdiagnosed by imaging examinations, for the intracavitary type, 7 cases were nodules cancer, and 1 case was infiltration cancer. And 4 cases out of 12 cases were miss diagnosis, the 4 cases were all linfiltration cancer and thick-walled carcinomas. Conclusion The linfiltration gallbladder cancer is easy to be missed, and the nodules gallbladder cancer is easy to be misdiagnosed. Improved understanding on the imaging signs of gallbladder diseases and used standard imaging examination techniques, the preoperative diagnosis rate of UGC will be improved.

Effect of peginterferon α-2a and Tenofovir Disoproxil on the expression of serum Mig, RANTES and indicators of hepatic fibrosis in newly diagnosed chronic hepatitis B patients with high viral load

Objective To investigate the effect of peginterferon α-2a combined with Tenofovir Disoproxil on serum monokine induced by interferon-γ (Mig), regulated upon activation normal T-cell expressed and secreted factors (RANTES) and indicators of liver fibrosis in newly diagnosed chronic hepatitis B patients with high viral load. Methods A total of 168 patients with HBeAg-positive chronic hepatitis B treated in West China-Guang′an Hospital, Sichuan University from June 2016 to December 2017 were selected and divided into the experimental group and the control group by random number table method, with 84 cases in each group. The patients in the control group received monotherapy with Tenofovir Disoproxil, on basis of which, the patients in the experimental group were added with peginterferon α-2a, and both groups were treated for 6 months. The therapeutic effects and serum Mig, RANTES and indicators of liver fibrosis were compared between the two groups. Results The HBV DNA negative rate, HbeAg negative rate and HBeAg seroconversion rate in the experimental group were significantly higher than those in the control group (P < 0.05). After treatment, the Mig and RANTES levels in the two groups were significantly lower than those before treatment (P < 0.05), and the Mig and RANTES levels in the experimental group were significantly lower than those in the control group (P < 0.05). After treatment, the indicators of liver fibrosis (hyaluronic acid, laminin, collagen-Ⅳ and procollagen-Ⅲ) in the two groups were significantly lower than those before treatment (P < 0.05), and the levels of indicators of liver fibrosis in the experimental group were significantly lower than those in the control group (P < 0.05). Conclusion Pegylated interferon α-2a combined with Tenofovir Disoproxil can significantly reduce the levels of serum Mig and RANTES in patients with hepatitis B, and effectively improve the indicators of liver fibrosis in patients with hepatitis B, which is worthy of clinical promotion.

“Top-down” and “bottom-up” methods were used to assess the measurement uncertainty of the same detection system for six tumor markers

Objective By evaluating the measurement uncertainty of six tumor markers, the applicability and differences of “top-down” and “bottom-up” evaluation methods in clinical laboratory were discussed. Methods Based on the Nordtest guideline for evaluating the uncertainty of the “top-down” approach, the internal quality control data of the six tumor markers were used as the source for introducing the uncertainty component of reproducibility in the laboratory. From 2015 to 2018, the return data of 6 external quality assessment of the ministry of health and the traceability data of calibration products were used as the source of the uncertainty component introduced by deviation, and the uncertainty of synthetic standard and relative standard were calculated. Guide to the expression of uncertainty in measurement (GUM) was used to assess the uncertainty of the “bottom-up” method. Reference laboratory technical reports, laboratory seven big sources of measurement uncertainty: repeatability of absorbance of equipment, temperature fluctuation of reagent chamber, temperature fluctuation of incubation plate, repeatability and accuracy of sampling system, system carrying pollution rate, fluctuation of environmental temperature and humidity, calibration of centrifuge was analyzed, and the synthesis standard uncertainty and relative uncertainty of each component were calculated. Results The relative synthesis uncertainty of the six tumor markers (α-fetoprotein, carcinoembryonic antigen, carbohydrate antigen 125, carbohydrate antigen 153, carbohydrate antigen 199 and total prostate-specific antigen) was assessed by the “top-down” method as 9.0%, 7.2%, 9.4%, 8.3%, 11.8%, and 9.8%, respectively. The results of the “bottom-up” method were 8.0%, 7.8%, 8.1%, 9.1%, 8.1% and 8.4%, respectively. There was no significant difference between the two methods (P > 0.05). Conclusion Both “top-down” and “bottom-up” methods can be applied to the evaluation of uncertainty in the measurement of tumor markers in clinical laboratories.

Co-word analysis of patient self-management based on PubMed database

To explore the research hotspots and trend of patient self-management, and provide thought and basis for further research. Methods From the construction of the database to September, 2018, all literatures related patients′ self-management from PubMed were retrieved and were objectively analyzed by bibliometric analytics (BICOMS) and co-word analysis method. The high-frequency subject headings of patients′ self-management were analyzed by co-occurrence bibliographic information mining system and SPSS 22.0. Results A total of 1809 related literatures were retrieved, and 58 high-frequency keywords were obtained. Through the analysis of high frequency keywords, it was found that the study of patient self-management gradually formed eight research hotspots: patient self-management efficiency, outcome relationships, physician/nurse-patient relationships, patient compliance, diabetes, drug self-management, self-care and educational research for chronic diseases, chronic diseases and telemedicine. Conclusion Patient self-management is in the stage of vigorous development, but the research level and characteristics need to be improved, and the standardization and systematization of patient self-management should be explored step by step.

Effect of palliative care management based on the theory of Knowledge-attitude-practic in the treatment of patients with advanced colorectal cancer chemotherapy

Objective To investigate the effect of palliative care based on the theory of Knowledge-attitude-practic on the improvement of pain degree and negative emotion in patients with colorectal cancer during chemotherapy. Methods Ninety-six patients with advanced colorectal cancer receiving chemotherapy from the First Affiliated Hospital of Wenzhou Medical University from March 2017 to March 2019 were selected for inclusion in the study. According to the random number table method, they were divided into the intervention group and control group, with 48 cases in each group. The patients in the control group were received routine care during chemotherapy, Patients in the intervention group were given palliative care care management based on the theoretical model of Knowledge-attitude-practic. The pain level, negative mood and nutrition level of patients in the two groups during chemotherapy were compared. Results After nursing for 30 d, visual analogue scale score and digital grading scale score of the two groups of patients were significantly reduced, and the intervention group was lower than the control group (P < 0.05). After 30 d of nursing care, the anxiety self-rating scale and depression self-rating scale scores of patients in the two groups were decreased, and the intervention group was lower than the control group (P < 0.05). After nursing for 30 d, the contents of total serum protein, albumin and transferrin in the two groups increased, and the contents in the intervention group were higher than those in the control group (P < 0.05). Conclusion Palliative care based on the theory of Knowledge-attitude-practic can alleviate the pain of colorectal cancer patients during chemotherapy, improve the patients′ negative mood and nutrition level, and improve the quality of life of patients, which is worthy of promotion.

The role of communication mode construction in outpatient nursing work

Objective To explore the role of communication mode in outpatient nursing work. Methods Based on the status quo of out-patient nursing, the communication mode was implemented for 32 out-patient nurses in Affiliated Hospital of Hangzhou Normal University (hereinafter referred to as “our hospital”). From January to February 2018, 100 outpatients in our hospital were selected as the control group for the satisfaction survey before the implementation of the communication mode. In addition, 100 outpatient patients in our hospital from May to June 2018 were selected as the observation group for the satisfaction survey after the implementation of the communication mode, and the communication ability and patient satisfaction of outpatient nurses before and after the implementation of the communication mode were compared. Results After the implementation of the communication mode, the scores of outpatient nurses in polite contact, warm introduction, detailed information and explanation, relationship inquiry, patient answer to questions, polite departure communication steps and overall communication skills were significantly improved compared with the scores before the implementation of the communication mode, with statistically significant differences (P < 0.05). The satisfaction of patients in the observation group was higher than that in the control group, and the difference was statistically significant (P < 0.05). Conclusion The communication mode has achieved satisfactory results in outpatient nurses, which can significantly improve the communication ability of outpatient nurses and patient satisfaction, which is worthy of clinical promotion.

Effect of KTH integrated nursing intervention on anxiety and depression of community stroke patients

To investigate the effect of KTH integrated nursing intervention on anxiety and depression in community stroke patients. Methods From January to December 2018, 92 cases of stroke patients in Wangfu Community, Xiangcheng City, Xiangyang City were selected as the study objects by convenient sampling method. They were divided into the observation group 47 cases and control group 45 cases by random number table method. The control group received routine nursing and health education, the observation group carried out KTH integrated nursing intervention on this basis, two weeks before and after the intervention, Zung anxiety self-rating scale (SAS) and depression self-rating scale (SDS) were used for comparison. Results There was no significant difference in SAS and SDS scores between the two groups before intervention (P > 0.05). After intervention, both groups had lower SAS and SDS scores than before intervention, and the observation group was lower than the control group, with statistically significant differences (all P < 0.01). Conclusion KTH integrated nursing intervention can significantly reduce anxiety and depression in community stroke patients.

Effect of systematic nursing intervention on urinary retention after total cerebral angiography

Objective To explore the effect of systemic nursing intervention on the incidence of urinary retention after digital subtraction angiography, and to provide reference for the clinical nursing of cerebrovascular diseases. Methods From May 2017 to May 2019, 265 patients underwent total cerebral angiography in the Department of Encephalopathy, Zhuhai Hospital of Guangdong Hospital of Traditional Chinese Medicine were selected, and the patients were divided into observation group 133 cases and control group 132 cases by random number table method. The control group received routine nursing, while the observation group carried out systematic nursing intervention measures such as psychological nursing, health education, and bed urination training on the basis of routine nursing. The incidence of urinary retention, urination function, complications and patient satisfaction were observed. Results The incidence of postoperative urinary retention in the observation group was significantly lower than that in the control group, and the urination function was significantly better than that in the control group, with statistically significant differences (P < 0.05). The incidence of complications in the observation group was significantly lower than that in the control group, and the difference was statistically significant (P < 0.05). The overall treatment satisfaction of the observation group was significantly higher than that of the control group, with statistically significant difference (P < 0.05). Conclusion Systematic nursing intervention can effectively improve the urination function of patients with digital subtraction angiography, reduce the incidence of urinary retention and other complications, and improve patients′ satisfaction with treatment.

Effect of group dynamics intervention in young and middle-aged hemodialysis patients

Objective To explore the application effect of group dynamics intervention in young and middle-aged hemodialysis patients. Methods A total of 120 young and middle-aged hemodialysis patients admitted to Hai an People′s Hospital in jiangsu province from March 2018 to March 2019 were selected, according to the random number table method was divided into control group and observation group, each group 60 cases. The control group was given regular health education, and the observation group was given health education with group dynamics intervention as the guide scheme. Biochemical indicators, post-traumatic growth score and quality of life were compared between the two groups. Results After 1 month of intervention, the serum albumin, serum phosphorus, serum calcium, serum potassium, creatinine and urea nitrogen levels in the observation group were all better than those in the control group (P < 0.05). The post-traumatic growth score of the observation group after 1 month of intervention was higher than that of the control group (P < 0.05). The whoqol-100 scale score of the two groups after 1 month of intervention was higher than that before intervention (P < 0.05). The WHOQOL-100 scale score in the observation group was higher than that in the control group after 1 month of intervention, with statistically significant differences (all P < 0.05). Conclusion The application of group dynamics intervention in young and middle-aged hemodialysis patients can effectively improve the level of serum albumin, blood phosphorus and other biochemical indicators, which is of great significance to improve the prognosis of patients, and is worthy of promotion.
