WANG Huiying1,2 LI Hongliang3 WANG Baoguo1
Objective To investigate the effectiveness and safety of stellate ganglion block (SGB) in the treatment of migraine and its influence on the hemodynamics of intracranial and cervical arteries. Methods Eighty patients met with the diagnostic criteria of migraine treated in Department of Pain, Sanbo Brain Hospital, Capital Medical University from June 2014 to January 2015 were selected. The pain degree was evaluated by numerical rating scale (NRS), the postoperative complications were recorded, and the efficacy was evaluated after operation for 6 months. The bilateral cervical arteries and intracranial arteries were detected by transcranial doppler sonography to record the indexes of hemodynamics. Results Compared with those before SGB, the scores of NRS after SGB were decreased, the difference was statistically significant (P < 0.05). The total effective rate of SGB after 6 months was 91.25%. Compared with that before block and the opposite block side, SGB could significantly increase the diastolic flow velocities of the block side of external carotid artery and common carotid artery, decrease the pulse index, the differences were all statistically significant (P < 0.05). The blood flow velocities, the changes of flow velocities and the pulse indexes of the block side and the opposite block side of the other cervical arteries and intracranial arteries had no statistically significant differences before and after SGB (P > 0.05). After SGB, among 80 cases of patients, there were 6 cases (7.50%) with hoarseness (completely disappeared within 1 h), 2 cases (2.50%) with transient upper limb numbness, 3 cases (3.75%) with brachial plexus block, 4 cases (5.00%) with puncture site pain, 3 cases (3.75%) with foreign body sensation. Conclusion SGB is efficacious and safe to treat migraine. SGB can relieve the symptoms of migraine may be achieved by lowering the vascular resistance of external carotid artery and common carotid artery, speeding up their blood flow velocity, and mildly lowering the vascular resistance of intracranial arteries.

29 Butein exerts pro-apoptotic activity in lung adenocarcinoma A549 cells via the regulation of STAT3 signaling
CHEN Shaoyang YUE Honglin LIU Xu
Objective To investigate the pro-apoptotic effect of Butein (BTN) in lung adenocarcinoma A549 cells and the role of STAT3 signaling in this process. Methods A549 cells were cultured and treated with different concentrations of BTN (0, 25, 50 μmol/L) for 24 h. BTN 0 μmol/L was taken as the control group. The survival rate, apoptotic rate, cell migration, ROS generation, Caspase-3 activity were respectively detected after treatment. Western blot was used to detect expression of p-STAT3, STAT3, Bcl-2 and Bax. NAC 5 mmol/L was used to block the generation of ROS, then BTN treatment (0, 50 μmol/L) was implemented for further detection as mentioned. Besides, in vivo studies were also used to verify whether abdominal injection with BTN (2, 4 mg/kg) exerted anticancer activity, while the control group was injected with PBS. Results Compared with the control group, BTN (25, 50 μmol/L) treatment resulted in reduction of cell viability and migration (P < 0.05), promotion of cell apoptosis (P < 0.05), up-regulation of ROS and Caspase-3 levels (P < 0.05). BTN treatment resulted in a significant down-regulation of p-STAT3 and Bcl-2, and up-regulation of Bax (P < 0.05). Moreover, inhibition of ROS by NAC antagonized the anti-STAT3 phosphorylated and pro-apoptotic role of BTN on A549 cells (P < 0.05). In vivo studies showed that compared the control group, tumor volume and p-STAT3 expression in nude mice were significantly reduced after BTN treatment (P < 0.05). Conclusion BTN treatment effectively inhibits cell viability and promotes apoptosis in A549 cells, which may be related to the up-regulation of ROS level thus inhibiting STAT3 phosphorylation.

57 Clinical research situation of asymptomatic cerebral infarction
WU Xiangju ZHANG Chunmei▲
Asymptomatic cerebral infarction (ACI) is a kind of cerebrovascular disease, which is associated with age, hypertension, carotid atherosclerosis and so on. Because of the ACI does not show typical symptoms and signs, both doctors and patients often ignore it, most of them are found by physical examination, at this time the patient's brain parenchyma has been damaged, and even irreversible pathological changes have happened. Early detection, intervention and treatment timely is extremely important. If not timely control the risk factors of ACI, it can increase the risk of cerebral infarction, neurocognitive dysfunction and mental illness. At present, ACI is detected mostly by MRI, high resolution MRI can improve the detection rate of ACI and reduce the misdiagnosis rate.

BAI Zunguang WANG Zhaohui DAI Ruixin ZHU Shoulun PAN Jun WU Tao
Objective To explore the anti-reflux effectiveness of direct ureteral nipple in laparoscopic ureteral reimplantation. Methods Thirteen patients (14 sides) underwent laparoscopic direct nipple ureteroneocystostomy. In each case, the lower end of the ureter was freed and delivered out of the peritoneal cavity through the ipsilateral trocar. Then the lateral ureter was tailored and folded back onto itself approximately 1 cm to make into nipple evaginated mode. A double-J stent was placed in the ureter and replaced in the abdomen. The posterolateral wall of bladder was opened. Then the nipple and double-J stent were placed into the bladder lumen and a mucosa-to-full thicknessanastomosis was made. Results Surgery was successful in all 13 patients. 13 sides monitored with cystourethrography and cystoscopy did not demonstrate vesicoureteral reflux. Cystoscopy revealed the intravesical nipple was 0.8-1.0 cm long. The shape of nipple was regular without depression and atrophy. But 1 case was revealed with third-degree reflux. Cystoscopy revealed the shape of nipple was irregular with partial depression and atrophy. The nipple remained open during the bladder-filling period. The median follow-up was 24 months (6-63 months). No patients showed the improvement of hydronephrosis or ureterectasia on ultrasound. No patients occurred acute pyelonephritis or the symptom of backache. Conclusion Laparoscopic ureteral reimplantation with direct ureteral nipple shows effective anti-reflux effectiveness. It is relatively easy to operate during the process to implant the nipple and worthy of being widely applied in clinic. However, such technical issues need to be standardized.

PENG Xiang1 LIU Yinghua2 CHEN Lianghua3 LIU Yuping2 LI Xiaoxia2 HUANG Jiongzhou2 YANG Weiwen2
After registration, patients are to be in procedure of ward round by doctors everyday clinically. Few doctors communicate with patients soul-by-soul even if they play guiding roles of direction during the ward round. In sequence, we propose a new pattern so called soul ward round in humanistic medicine developed by sympathetic technique based on humanistic care. During this soul ward round, patients usually act as principals of depiction, while doctors act as soul helpers accompanying patients to let them narrate more. The synopsis and method includes self-sympathy, patient-sympathy and sympathy on suitable options. Through soul ward round practiced by these techniques, the doctors feel patients' most real perception by heart, and provide care and assistance so that the patients are courage by confidence while considering themselves to be primary act, and the medical pattern of patient-centralized based on human being is realized in the reality.

Methods The databases of PubMed, Medline, CBM disc, Cochrane library, CNKI, VIP, Wanfang, China Master's Theses Full-text Database and proceedings of the academic conference were retrieved. The retrieval time limit was from January 2000 to October 2016. The related clinical randomized controlled trials were researched, and the enrolled literatures were taken quality assessment and data extraction, and Revman5.3 software was used to take Meta analysis. Results In total, 26 clinical randomized controlled trials and 2421 patients were enrolled, with 1307 cases in experimental group and 1114 cases in control group. The levels of whole high blood viscosity (200/s) (SMD = -0.63, 95%CI: -0.91 - -0.34, P < 0.01), whole low blood viscosity (5/s) (WMD = -0.76, 95%CI: -1.02 - -0.50, P < 0.01), plasma viscosity (SMD = -0.62, 95%CI: -1.02 - -0.22, P = 0.002), fibrinogen (SMD = -0.67, 95%CI: -0.80 - -0.54, P < 0.01), red blood cell aggregation index (SMD = -0.96, 95%CI: -1.46 - -0.47, P = 0.0001) in the experimental group were significantly lower than those of control group, the fundus changing curative effect (RR = 1.22, 95%CI: 1.17 - 1.28, P < 0.01) and TCM syndrome curative effect (RR = 1.55, 95%CI: 1.18 - 2.03, P = 0.002) were significantly better than those of control group. The whole middle blood viscosity (30/s) between two groups had no statistically significant difference (MD = -1.22, 95%CI: -2.66 - 0.21, P = 0.10). Sensitivity analysis showed that the results were stable and reliable. Conclusion Traditional Chinese medicine treatment can reduce the whole high blood viscosity (200/s), whole low blood viscosity (5/s), plasma viscosity, fibrinogen, erythrocyte aggregation index of patients with diabetic retinopathy, and improve the fundus changing curative effect and TCM syndrome curative effect, all of which are superior to those of single western medicine treatment, while it has no effect on the whole middle blood viscosity (30/s). This paper shows that traditional Chinese medicine treatment for diabetic retinopathy has certain creative effect, but the methodological quality remains to be further improved, which needs more high-quality randomized, double-blind, controlled literatures to confirm.
