Methods A total of 45 SPF male SD rats, 6-8 weeks old, weighing 150-180 g were selected, and 15 rats were selected as Sham operation group, the rest were established by 2-vessel occlusion (2VO) to establish the chronic cerebral ischemia model. After successful modeling, the Sham operation animals were divided into three groups, and the model animals were divided into six groups by random number table method, with five animals in each group. The specific groups were modeling 1 month group (Sham group, 2VO group, 2VO+CIG60 group, 2VO+CIG120 group, 2VO+ Butylphthalide group); modeling 1.5 months group (Sham group, 2VO group); modeling 3 months group (Sham group, 2VO group). After six hours of modeling, CIG 60 mg/(kg·d) was gavaged in 2VO+CIG60 group, CIG 120 mg/(kg·d) was gavaged in 2VO+CIG120 group, Butylphthalide 100 mg/(kg·d) was gavaged in 2VO+Butylphthalide group, and Sham group and 2VO group were gavage with the same amount of normal saline. At 1, 1.5, and 3 months after modeling, the hypothalamus and midbrain of rats were taken to detect the levels of clock genes CRY1, PER1, PER2, and NPAS2 mRNA, the changes of clock genes were observed. Results In hypothalamus, compared with Sham group, PER2 mRNA in 2VO group was decreased at 1 month after modeling (P<0.01); clock gene PER2 mRNA and NPAS2 mRNA in 2VO group were increased at 1.5 months after modeling (P<0.05 or P<0.01). In midbrain, compared with Sham group, CRY1 mRNA and PER1 mRNA in 2VO group were higher at 1 month after modeling (P<0.05 or P<0.01); PER2 mRNA in 2VO group was increased at 3 months of modeling (P<0.05). In hypothalamus, compared with Sham group, PER2 mRNA in 2VO group was decreased (P<0.01); compared with 2VO group, NPAS2 mRNA in 2VO+CIG60 group was increased, PER1 mRNA in 2VO+CIG120 group was decreased (P<0.05). In midbrain, compared with Sham group, CRY1 mRNA and PER1 mRNA in 2VO group were increased (P<0.05 or P<0.01); compared with 2VO group, CRY1 mRNA in 2VO+CIG120 group was decreased (P<0.01). Conclusion Disordered expression of clock genes in the hypothalamus and midbrain of vascular dementia model rats causes disruption of the feedback loop that regulates circadian rhythm, and CIG can correct the disordered expression of clock genes in the hypothalamus and midbrain.

ZHAO Jun1 LI Xia1 TIAN Junjian1 WANG Xin2 WANG Xuefei2 LI Zhigang1
DOI: 10.20047/j.issn1673-7210.2023.08.07
Irritable bowel syndrome (IBS) is a chronic functional gastrointestinal disease. It is not life-threatening, but affects the quality of life and increases the risk of psychiatric disorders such as depression. Therefore, it is necessary to explore the brain central pathogenesis of IBS. Functional magnetic resonance imaging is a very effective and non-invasive in vivo observation technique for studying brain function, which provides a new perspective for evaluating the abnormality of “brain-gut axis” in IBS. Although the efficacy of acupuncture-moxibustion on IBS are positive, their brain central mechanisms are still unclear. This article reviews relevant domestic and foreign literature of acupuncture-moxibustion in recent years, and summarizes the application of functional magnetic resonance imaging technology in IBS.

LIU Yulin1 ZHANG Gengchao1 LIAO Chengcheng2 XIANG Mingli2 LIU Jianguo3 BAI Ding4 GUAN Xiaoyan2
DOI: 10.20047/j.issn1673-7210.2023.08.08
Low-intensity pulsed ultrasound (LIPUS) is an economical and safe physical therapy, and usually used to treat bone discontinuities and tissue healing. Mesenchymal stem cells (MSCs) are important seed cells in regenerative medicine with high self-renewal and multi-directional differentiation ability. The combined application of LIPUS and MSCs can regulate the proliferation, migration, cell activity, multi-directional differentiation and other abilities of MSCs, effectively promote the regeneration of MSCs in bone, cartilage, blood vessels, nerves, and other tissues, and inhibit the regeneration of adipose tissue. Therefore, this article reviews the mechanism of LIPUS regulating the repair and regeneration of MSCs in different tissues in recent years, provides theoretical basis for the continuous research and clinical transformation of LIPUS in the field of tissue regeneration.

CHENG Fang1 HU Kunmin1 ZHU Shan2
DOI: 10.20047/j.issn1673-7210.2023.08.09
Blood-activating and stasis-resolving traditional Chinese medicines are mainly used for blood stasis syndrome of the pain of internal medicine, amenorrhoea, dysmenorrhea, traumatic injury, sore and ulcer, swelling, and so on. Inflammation is the natural response of the body to various injury stimuli. Blood-activating and stasis-resolving traditional Chinese medicine has certain advantages in inhibiting inflammatory reaction. This paper analyzes the anti-inflammatory mechanism of nine traditional Chinese medicines including Corydalis Rhizoma, Chuanxiong Rhizoma, Salviae Miltiorrhizae Radix Et Rhizoma, Curcumae Radix, Olibanum, Leonuri Herba, Achyranthis Bidentatae Radix, Strychni Semen, and Hirudo by searching the literature. It is found that blood-activating and stasis-resolving traditional Chinese medicine mainly plays an anti-inflammatory role by three signal pathways of nuclear factor- κB, mitogen-activated protein kinase, nuclear transcription factor E2 related factor 2-antioxidant response element. It also provides research direction for the existing problems in this research field, and provides reliable theoretical basis for further exploring the anti-inflammatory component target and clinical application of blood-activating and stasis-resolving traditional Chinese medicines.

WU Yiwei ZHANG Kecheng SUN Yunwei WU You CHEN Yan
DOI: 10.20047/j.issn1673-7210.2023.08.10
There is bidirectional association between obstructive sleep apnea hypopnea syndrome (OSAHS) and metabolic diseases such as obesity, type 2 diabetes mellitus, and nonalcoholic fatty liver. In recent years, the incidence rate of OSAHS continues to rise, causing serious harm to both physical and mental health of patients. However, in previous studies, the mechanism of action related to OSAHS and metabolic diseases is not clear enough, and the treatment methods of OSAHS combined with metabolic diseases are also relatively limited. In recent years, OSAHS and metabolic diseases become research hotspots. Significant progress is made in the study of their common pathogenesis, and weight loss treatment is also gain attention. Therefore, this paper reviews the latest advances in the relationship between OSAHS and metabolic diseases and their intervention, from the aspects of incidence rate, common mechanism, treatment and so on, aiming at helping the progress of clinical treatment of metabolic diseases in OSAHS.

Research progress of human epidermal growth factor receptor in metastatic colorectal cancer
REN Yanzhen LI Ruixian
DOI: 10.20047/j.issn1673-7210.2023.08.12
The incidence of human epidermal growth factor receptor 2 (HER2) variants is low in patients with metastatic colon cancer (mCRC), and there is no uniform standard for detection methods. HER2 mutations can lead to resistance to anti-epidermal growth factor receptor-targeted therapy in mCRC. Single anti-HER2-targeted therapy has limited effect, however, dual anti-HER2-targeted therapy such as Trastuzumab combined with Lapatinib or Pertuzumab has shown good anti-tumor efficacy in mCRC. This article aims to review the detection, clinicopathological features, predictive value, and treatment strategies, and prospects of HER2 in mCRC.

DOI: 10.20047/j.issn1673-7210.2023.08.18
Objective To investigate the significance of serum neuron-specific enolase (NSE) in evaluating the severity and short-term prognosis of acute cerebral infarction patients. Methods A total of 126 patients diagnosed with acute cerebral infarction in the Department of Neurology, Baoshan Branch, Renji Hospital, Shanghai Jiao Tong University School of Medicine (hereinafter referred to as “our hospital”) from January 2019 to April 2020 were selected as the acute cerebral infarction group, and 55 physical healthy subjects in our hospital during the same period were selected as the control group. The clinical data of acute cerebral infarction group and control group were compared. The acute cerebral infarction group was classified as mild (46 cases), medium (47 cases), and severe (33 cases) according to the score of the National Institutes of Health stroke scale (NIHSS). Then, according to the degree of neurological impairment at discharge, the patients were divided into significant improvement (41 cases), improvement (40 cases), and no change (45 cases). The difference of serum NSE level in patients with different severity and short-term prognosis was compared, and the correlation between NSE level and the score of NIHSS, the neurological deficit in acute cerebral infarction group was analyzed. The area under the receiver operating characteristic curve (AUC), optimal critical value, sensitivity, and specificity of NSE in determining the severity and short-term prognosis of patients with acute cerebral infarction were evaluated. Results The level of NSE in acute cerebral infarction group was higher than that in control group, and the difference was statistically significant (P<0.05). The level of NSE in mild, moderate, and severe patients was increased gradually, and the level of NSE in patients with significant improvement, improvement, and no change was increased gradually, the differences were statistically significant (P<0.05). The level of NSE in acute cerebral infarction group was positively correlated with NIHSS score (r>0, P<0.05), and negatively correlated with the score of neurological deficit (r<0, P<0.05). Serum NSE level had a certain value in predicting the severity and short-term prognosis of patients with acute cerebral infarction (AUC>0.5, P<0.5). Conclusion Serum NSE level is valuable for evaluating the severity of central nervous system injury and short-term clinical prognosis of patients with acute cerebral infarction, and can be used as a reliable biomarker for evaluating the severity of acute cerebral infarction and short-term clinical prognosis.

DOI: 10.20047/j.issn1673-7210.2023.08.23
Objective To analyze the correlation between the miR129-1 expression level in peripheral blood and prognosis in patients with non-small cell lung cancer (NSCLC). Methods A total of 109 patients with NSCLC admitted to the 960th Hospital of Chinese People’s Liberation Army and followed up for three years from January to December 2017 were selected as the study objects. According to the prognosis, they were divided into death group and survival group. The clinical case characteristics of the two groups were compared, and the factors influencing the prognosis of NSCLC patients were analyzed. The survival of patients with low and high expression of miR129-1 in peripheral blood was compared. Results Of the 109 patients, seven were lost to follow-up, 37 died, and 65 survived among the remaining 102 patients. The proportion of smoking history, lymph node metastasis, low differentiation, stage Ⅲ, and low expression of miR129-1 in peripheral blood in the death group was higher than those in the survival group, the differences were statistically significant (P<0.05). Multivariate analysis showed that lymph node metastasis, degree of differentiation, clinical stage, and peripheral blood miR129-1 expression were all influencing factors of prognosis in NSCLC patients (P<0.05). The 3-year survival rate of patients with low expression of miR129-1 in peripheral blood was lower than that of patients with high expression, and the difference was statistically significant (P<0.05). Conclusion Peripheral blood miR129-1 expression level is closely related to the prognosis of patients with NSCLC and is expected to be a biomarker for the diagnosis and treatment of NSCLC.

DOI: 10.20047/j.issn1673-7210.2023.08.29
Objective To investigate the factors influencing catheter-associated infections (CRI) in hemodialysis patients. Methods Four hundred and eighty patients who underwent hemodialysis from January 2018 to December 2021 in the Department of Nephrology, the Fifth Medical Center, Chinese People’s Liberation Army General Hospital were selected, the CRI rate of patients was recorded, and the CRI rate of patients with different clinical characteristics was compared. Multi-factor analysis was used to determine the influencing factors of CRI. Results A total of 46 patients out of 480 patients developed CRI, with an infection rate of 9.58%. There were 17 tunnel infections (36.96%), 14 catheter colonization infections (26.67%), 9 exit infections (19.57%), and 6 bloodstream infections (13.04%). There was a statistically significant difference in the rate of CRI in hemodialysis patients with different age, number of punctures, duration of catheter retention, combined diabetes mellitus and hypoproteinemia, length of hospital stay, and application of antimicrobial drugs (P<0.05). Multifactorial logistic regression analysis showed that, age (OR=2.254), diabetes (OR=2.315), hypoproteinemia (OR=3.015), number of punctures (OR=2.708), duration of indwelling catheter (OR=2.937), length of hospital stay (OR=2.830), and antimicrobial drug application (OR=2.401) were blood purification influencing factors of CRI in patients (P<0.05). Conclusion Patients with older age, combined diabetes mellitus and hypoproteinemia, more invasive operations, longer hospital stay, and not using antimicrobial drugs are more prone to CRI, so the clinic should pay attention to the management of comorbidities in hemodialysis patients and reduce invasive operations and use antimicrobial drugs reasonably to reduce CRI.

Discussion on regulative effects of Zuogui Pills and Yougui Pills on immune system from the theory of vital gate water and fire
ZOU Jixuan1 ZHAO Pei1 SONG Ying2 CAO Xintian1 WANG Mingjing2 LIU Weiyi2 LYU Yan2 HU Xiaomei2
DOI: 10.20047/j.issn1673-7210.2023.08.31
“Immunity” is a defense mechanism used by the body to resist foreign bodies and recognize and deal with abnormal cells. It is highly similar in function to “yang qi” in traditional Chinese medicine. And yang qi mainly originates from the vital gate. Zuogui Pills and Yougui Pills, created by Zhang Jingyue based on his theory of vital gate water and fire, are famous prescriptions for regulating and supplementing zhenyin and yuanyang of vital gate. Therefore, based on the theory of vital gate water and fire, combined with modern research progress, this paper discusses the regulatory effects of Zuogui Pills and Yougui Pills on immune system. It is found that the functions of the vital gate are highly similar to those of the central immune organs. It can be said that the zhenyin of the vital gate is the concentrated embodiment of the material basis of immune activities, and the bioactive molecules such as cytokines secreted by various cells in the bone marrow are the specific manifestations of the vital gate yuanyang. Zuogui Pills and Yougui Pills can regulate the vital gate water and fire, restore the imbalance of yin and yang to the equilibrium state of mutual root and mutual use, and play a role in regulating the body’s immune system disorder. It is feasible to use Zuogui Pills and Yougui Pills to treat some autoimmune diseases with deficiency syndrome as the main syndrome, and it is worth further exploration and popularization.

DOI: 10.20047/j.issn1673-7210.2023.08.33
Traditional knowledge of traditional Chinese medicine is a subordinate concept of traditional knowledge, which has the basic characteristics of subject diversity, inheritance, regionality, personal dependence, and tradition. It includes static traditional knowledge of traditional Chinese medicine and active traditional knowledge of traditional Chinese medicine. Among them, static knowledge refers to a certain form of solidified knowledge set, such as traditional medical literature, medical instruments, etc. Active knowledge refers to the knowledge set with potential improvement space, such as traditional Chinese medicine theory, secret recipe, diagnosis and treatment technology, processing technology, health care technology, etc. The protection of traditional knowledge of traditional Chinese medicine is different from intangible cultural heritage and intellectual property rights. While protecting cultural traditions and economic interests, it also protects the existing conditions of traditional knowledge. The feasible schemes for the protection of traditional Chinese medicine include legal means, administrative means, and intellectual property means to confirm the intellectual property rights of the above-mentioned static and active traditional knowledge of traditional Chinese medicine at the national, industrial, and social levels, prevent improper possession, promote and publicize, and inherit and develop.

Wang Qingguo’s experience in treating palpitation based on the combination of “disease, syndrome, and symptom”
LI Ting1 TANG Xuechun2 SUN Zijin2 OUYANG Huan2 CHENG Fafeng2 WANG Qingguo2
DOI: 10.20047/j.issn1673-7210.2023.08.34
Palpitation is one of the common clinical diseases, which seriously affects the physical and mental health and interferes with the life and work of patients. Meanwhile, traditional Chinese medicine treatment plays a particular therapeutic role. Professor Wang Qingguo, who has been conducting medical practice for more than 50 years and has rich clinical experience, recommends the diagnosis and treatment strategy by organically combining “disease, syndrome, and symptom”, so that doctors could grasp the main contradiction of palpitation, predict the trend of the disease evolution and use targeted drugs to minimize the patient’s suffering. This article introduces Professor Wang Qingguo’s understanding of the etiology and pathogenesis of palpitation and summarizes his academic thinking and commonly used prescriptions from three aspects: combining Chinese and Western medicine to identify the disease, identifying the syndrome and choosing the therapy based on the outline of deficiency and excess, and adjusting the medication prescription to restore the heart rhythm according to the symptoms. Besides, a typical clinical case is also attached for reference.

Experience summary of Professor Hu Guoheng in treating impediment disease based on collateral disease theory
XIE Jinru1 CHEN Ya2 HU Guoheng3
DOI: 10.20047/j.issn1673-7210.2023.08.35
Impediment disease is a common clinical chronic disease and senile disease, often with pain as the main symptom. Professor Hu Guoheng has rich experience in the diagnosis and treatment of this disease. On the basis of the understanding of “collateral disease theory”“treatment of impediment disease using collateral theory”, and “viscera deficiency and collateral obstruction”, he believes that collateral obstruction should be based on “yin collaterals”, the root of impediment disease is deficiency of kidney-essence and qi, and obstruction of kidney collaterals, the key to the pathogenesis is the deficiency of heart-qi and blood, and the obstruction of heart collaterals, and the pathological basis is the obstruction of the heart and kidney collaterals, which is blocked by phlegm and blood stasis. In terms of treatment, emphasis is placed on nourishing the heart and kidney, regulating qi and blood, and advocating the combination of rigidity and softness on the principles of diffusing impediment and dredging collateral, resolving phlegm and dispelling blood stasis. The clinical effect is quite good. It verifies the feasibility and effectiveness of treating impediment disease using collateral theory from “heart and kidney”. Therefore, this article introduces Professor Hu’s clinical experience in impediment disease combined with a clinical medical case.

Exploring the application of machine learning in predicting the effect of acupuncture for insomnia based on brain functional connectivity
YIN Xuejiao1 JIANG Tongfei1 CHEN Zhaoyi1 SONG Zhangxiao2 LI Bin1 GUO Jing1
DOI: 10.20047/j.issn1673-7210.2023.08.43
Acupuncture is one of the effective ways to treat insomnia, but because it is based on clinical experience, the treatment protocol has yet to be optimized, and the efficacy is mainly subjective assessment, with significant differences in individual efficacy. Therefore, it is important to screen effective populations for individual identification markers to predict efficacy and optimize acupuncture treatment protocols. Machine learning (ML) for classification and prediction algorithms are rapidly becoming tools for disease diagnosis, prediction, and classification. In contrast, magnetic resonance imaging has revealed that the brain functional connectivity (FC) of insomniac individuals is highly specific, with specific patterns of change before and after acupuncture, and can be used as a marker to track treatment response. This paper proposes to explore the feasibility of using the classification algorithm model in ML to predict the efficacy of acupuncture treatment for insomnia based on FC technology, in order to break the bottleneck of “empirical treatment” and optimize the acupuncture treatment plan.
