7 Visual analysis of research hotspots and trends of Buyang Huanwu Decoction based on CiteSpace

Objective To visually analysis the research hotspots, fronts and trends related to Buyang Huanwu Decoction based on CiteSpace. Methods Literatures related to buyang Huanwu Decoction published in the database of China National Knowledge Network from January 2011 to April 2021 were searched, meanwhile, the trend of publication, author, institution, keyword co-occurrence, and clustering were processed and analyzed respectively, and the network knowledge map was drawn. Finally, the visual analysis results were combined with manual retrieval for in-depth discussion. Results A total of 4 163 related literatures were included. In recent ten years, the number of Buyang Huanwu Decoction related literatures had been on the rise. The scholars who published the most articles are Zhang Jiping and Cai Guangxian, of which Cai Guangxian the team working most closely. Institutional analysis showed that Hunan University of Chinese Medicine issued the most articles. The high-frequency keywords were cerebral infarction, Qi deficiency and blood stasis, clinical efficacy, neurological function, hemorheology, etc. Cerebral infarction, idiopathic pulmonary fibrosis, cell apoptosis, and other clusters were formed. In the past five years, the main breakout words were therapeutic effect, autophagy, nervous function, ischemic stroke, motor function, hemiplegia, etc. In recent years, the research focus in this field was mainly on the molecular level of mechanism intervention and action targets. Conclusion Buyang Huanwu decoction is mainly used for the prevention and treatment of cerebrovascular diseases, pulmonary fibrosis and other diseases in clinical practice. Cytobiology, molecular biology, and other techniques are the trend in this field in recent years, with a large research potential, and should take the combination of Chinese and western as the guiding ideology to explore its potential value and better serve the clinic.

13 Study on medication rules of stroke based on the collation of ancient medical records

Objective To explore the medication rules of ancient medical records in stroke. Methods From December 2019 to December 2021, A total of 68 kinds of ancient books of traditional Chinese medicine medical records were selected, the prescription information of stroke medical records was extracted from medical records using Python3.8 programming language, input into Excel 2019, and the prescription data was normalized processing, statistical description, association analysis, and visual display. SPSS Statistics 26.0 software to complete the system cluster analysis. Results A total of 499 prescriptions were included in the first medical records, involving 302 types of drugs, with a total drug use frequency of 5 178 , and 32 kinds of high-frequency drugs (frequency ≥45). The drug with the highest frequency was Poria, which was 244 times, followed by Glycyrrhizae Radix Et Rhizoma, Pinelliae Rhizoma, Angelicae Sinensis Radix, Ginseng Radix Et Rhizoma, Atractylodis Macrocephalae Rhizoma, and so on. The drug natures of high frequency drugs were mainly warm, cold, and plain, and the flavours of drugs was mainly sweet, bitter, and pungent, and the drugs were mainly channeled into the spleen, lung, heart, stomach, liver, and kidney channels. A total of ten groups of core drug combinations were obtained by association analysis. The 32 high frequency drugs were divided into seven categories by systematic cluster analysis. Conclusion The drugs used in ancient medical records of stroke mainly focus on invigorating spleen and benefiting qi, eliminating phlegm and extinguish wind, regulating liver and kidney, emphasizing both strengthening vital qi to eliminate pathogenic factor, giving consideration to both manifestation and root cause, which can be used for clinical reference.

17 Data mining analysis of the medication pattern in the treatment of cerebral diseases in Ye Tianshi’s Clinical Guide of Medical Records

Objective To analyze the medication pattern of brain diseases in Ye Tianshi’s Clinical Guide to Medical Cases, and to provide reference for the clinical treatment of brain diseases in Chinese medicine. Methods The medical cases for the treatment of brain disorders in the Clinical Guide of Medical Records was screened, the drugs in the prescriptions of the cases were standardized and entered them into the Traditional Chinese Medicine Heritage Assist Platform (V2.5), and the four qi, five tastes, the attribution, the frequency of use, and the common pairs of drugs in the entered prescriptions were analyzed, and the complex system entropy clustering algorithm to generate new prescriptions were used. Results A total of 116 prescriptions containing 151 herbal medicines were recorded, among which the proportion of cold and sweet medicines was the largest, and the liver meridian was the main attribute, while the more frequently used herbal medicines were Poria, Radix et Rhizoma, Ginseng, Lycium, and Chrysanthemum. Twenty-four sets of association rules with support ≥8 and confidence ≥ 0.5 were obtained, with correlation degree was 10, penalty degree was 4 and association coefficient ≥0.023, and 26 pairs of medicine pairs were obtained based on the improved mutual information method of mining, while 11 new prescriptions were obtained. Conclusion Ye Tianshi’s treatment of brain disorders emphasizes treatment from the kidney and filling the marrow, and is good at using warming and phlegm-transforming herbs, and the new prescriptions obtained are of certain clinical application value.

22 Molecular basis of the rabbit model of combination of disease and syndrome and cold-dampness Bi syndrome knee osteoarthritis based on plasma metabolomics

Objective To explore the scientific nature of rabbit model of combination of disease and syndrome and cold-dampness Bi syndrome knee osteoarthritis based on rabbit plasma metabolomics. Methods Thirty healthy 4-month-old New Zealand rabbits were divided into blank group (group A), model group (group B), and cold-dampness group (group C), with 10 rabbits in each group. Group A did not do any treatment; in group B, plaster was applied to the knee joint to the ankle joint for four hours, repeated for six weeks; in group C, the knee joint was immersed in water at (6.0±0.5)℃ every day for four hours, and then the plaster was applied to the knee joint and ankle joint for four hours, repeated for six weeks. The differential metabolites between groups were screened and KEGG differential metabolic pathways were analyzed. Results A total of 74 and 25 differential metabolites was screened from group A-B and group B-C (VIP>1, P<0.05), respectively. Group A-B mainly involved in tryptophan metabolism, group B-C mainly involved in biliary secretion metabolism, 5-hydroxytryptamine metabolism, and arachidonic acid metabolism. Conclusion The rabbit model of combination of disease and syndrome and cold-dampness Bi syndrome knee osteoarthritis is related to the cold-dampness Bi syndrome knee osteoarthritis in traditional Chinese medicine disease and syndrome in terms of inflammatory response and lipid metabolism.

27 Influence of Anemoside B4 on ulcerative colitis in mice based on regulation of macrophage polarization

Objective To investigate the effect of Anemoside B4 on ulcerative colitis (UC) induced by dextran sulfate sodium (DSS) in mice based on regulation of macrophage polarization. Methods Forty SPF male C57BL/6 mice (body weight 18-20 g) were divided into control group, model group, B4 group, and positive drug group by random number table method, with ten mice in each group. Model group, B4 group, and positive drug group were given 3%DSS for five days consecutively to construct UC model. After successful modeling, B4 group was given intragastric administration of Anemoside B4 (5 mg/kg, once a day), positive drug group was given intragastric administration of Mesalazine (400 mg/kg, once a day), control group and model group were given intragastric administration of the same amount of normal saline. All four groups were given continuous intervention for seven days. The changes of mice body weight during the intervention were observed. After the intervention, the colon tissues of mice were stained with hematoxylin-eosin, and the histopathological index was further evaluated, and the inflammatory indicators of four groups were detected. The mRNA expression levels of nuclear factor-κB (NF-κB) p50, p65, and M1 macrophages type specific index inducible nitric oxide synthase (iNOS), M2 macrophages type specific index arginine-1 (Arg1) in colon tissues of the four groups were detected by qRT-PCR. The polarization of macrophages in the colon tissues of the four groups was observed by immunohistochemical staining. Results The body weight of the model group was lower than those of the control group at 6, 9, and 12 d after intervention, while the body weight of the B4 group at 12 d after intervention was higher than that of the model group (P<0.01 or P<0.05). In the control group, mucosal intestinal glands were arranged neatly without ulcerative lesions. In the model group, most of the crypts and epithelium of the intestinal mucosal layer were lost, accompanied by a large number of inflammatory cell infiltration, inflammatory invasion of the mucosal layer and submucosa. In B4 group, the colonic mucosa had slight inflammation, ulcer, and bleeding symptoms were significantly reduced, the epithelium and recess were relatively intact, and the colonic mucosa injury was also significantly improved. Histopathological index, levels of tumor necrosis factor-α (TNF-α), interleukin (IL)-1β, IL-6, and mRNA expression levels of NF-κB p50, NF-κBp65, iNOS in model group were higher than those in control group, levels of IL-10, transforming growth factor-β (TGF-β), and mRNA expression level of arginine-1 (Arg1) were lower than those in control group (P<0.05). Histopathological index, levels of TNF-α, IL-1β, IL-6, and mRNA expression levels of NF-κBp50, NF-κBp65, iNOS in B4 group were lower than those in model group, levels of IL-10, TGF-β, and mRNA expression level of Arg1 were higher than those in model group (P<0.05). The proportion of M1 macrophages in the model group was higher than that in the control group, and the proportion of M2 macrophages was lower than that in the control group, the proportion of M1 macrophages in the B4 group was higher than that in the model group, and the proportion of M2 macrophages was higher than that in the model group (P<0.05). Conclusion Anemoside B4 can inhibit the M1 macrophages, promote the M1 macrophages, improve the intestinal inflammatory environment and relieve UC.

32 Detection of three polymorphic sites of CYP2C19 gene * 2, * 3, and * 17 based on TaqMan-MGB probe technology

Objective To establish a method for detection of the three polymorphic sites * 2, * 3, and * 17 of CYP2C19 based on TaqMan-MGB probe technology. Methods Corresponding primers and double-labeled probes were designed for * 2, * 3 and * 17 sites of CYP2C19 gene. The remaining whole blood samples from the outpatient Department of Cardiology, Guizhou Provincial People’s Hospital from March 2021 to June 2022 with interested genotype were selected. The 42 samples containing 10 genotypes at each site were collected and the whole blood genomic DNA was extracted by polymerase chain reaction (PCR) detection, followed by electrophoresis and Sanger sequencing. 1, 10, 100, 800 ng DNA was added and PCR detection was conducted to evaluate the sensitivity. Five cases of each genotype of each locus were detected once a week for three consecutive weeks, to evaluate the repeatability. Results The designed primers successfully amplified the target gene fragment and the electrophoresis showed a bright single DNA band; the PCR results of 90 genotypes were consistent with the sequencing results (P>0.05); the genotypes could be accurately identified by PCR when different amount of DNA was added and the Ct values of all genotypes at 1 ng level were less than 34. The results of three repeated tests for the same genotype were consistent. Conclusion This study successfully establish a TaqMan-MGB probe to detect the three polymorphic sites, * 2, * 3, and * 17 of CYP2C19 gene. The method is fast, accurate, sensitive and simple.

37 Research progress of new Bruton’s tyrosine kinase inhibitor Orelabrutinib

Orelabrutinib, a novel Bruton’s tyrosine kinase (BTK) inhibitor, is a class Ⅰ innovative drug independently developed in China. Its emergence brings new therapeutic methods for the treatment of lymphoma patients, especially the patients with relapsed/refractory mantle cell lymphoma and relapsed/refractory chronic lymphocytic leukemia/small cell leukemia. BTK is a key kinase in the B-cell antigen receptor signaling pathway and is crucial to the proliferation and survival of malignant B cells, thus it is a promising target for the treatment of B-cell malignant lymphoma. Therefore, this paper reviews the mechanism of action of BTK inhibitor and the basic information of listed BTK inhibitors, summarizes the pharmacological properties and clinical data of Orelabrutinib, and sorts out the latest clinical research progress of Orelabrutinib so far, and proposed that Orelabrutinib is the most potential BTK inhibitor at present and candidate drug for BTK inhibitor combination therapy.

42 Research progress on chemical constituents and pharmacological activities of Saccharum Alhagi

Saccharum Alhagi is a kind of traditional Uyghur ethnic medicine, which is a sugar granule secreted and condensed from the branches and leaves of the leguminous plant Alhagi Sparsifolia Shap. The use of Saccharum Alhagi in Chinese medicine has a long history. Compendium of Materia Medica records that Saccharum Alhagi has the functions of nourishing and strengthening, balancing body fluids, regulating endocrine and astringent intestinal pain, expectorating cough, and so on. Saccharum Alhagi contains a large number of polysaccharides, oligosaccharides, flavonoids, volatile oil, amino acids, trace elements, and other chemical components. With the continuous study of its pharmacological activity, it has been found that the extract of Saccharum Alhagi has the activities of immune regulation, liver and kidney protection, bacteriostatic, antioxidant, hypoglycemic, double regulation of intestinal smooth muscle, and so on. In this paper, the research progress of chemical constituents and pharmacological activities of Saccharum Alhagi in recent years is reviewed in order to provide new ideas and references for further development and clinical research of Saccharum Alhagi.

46 Clinical research progress in the treatment of herpes zoster with fire needle and cupping

By observing the clinical effects of fire needle and cupping therapy on herpes zoster in the past ten years, this paper concludes that fire needle and cupping therapy on herpes zoster has significant curative effect, which is reflected in good analgesic effect, faster healing speed of herpes, and reduced incidence of posthumous neuralgia and so on. However, there are still some problems in the current clinical treatment of herpes zoster, and it is urgent to further carry out large-scale high-quality randomized controlled trials with standardized design, scientific design, and multi-dimensional efficacy evaluation, which will boost the formation of evidence-based supporting evidence of traditional Chinese medicine, increase the influence of traditional Chinese medicine in the medical level, promote the steady development of traditional Chinese medicine, and facilitate the popularization and promotion of the treatment.

50 Research progress of epidermal growth factor receptor targeted therapy for cervical cancer

The occurrence of cervical cancer is generally associated with human papillomavirus (HPV) infection, and there was a strong correlation between HPV infection and epidermal growth factor receptor (EGFR) signaling cascade upregulation. Therefore, EGFR is commonly targeted and used to treat cervical cancer using tyrosine kinase inhibitors and monoclonal antibodies. This review aims to integrate contemporary approaches to EGFR-targeted therapy of cervical cancer and highlight the current differential roles of microRNA and associated genes in cervical cancer involving EGFR mutations. Potential resistance to existing EGFR therapies and the need for better therapies are reviewed and discussed with a view to developing novel agents with better efficacy.

54 Advances in the application of transcranial magnetic stimulation in central neuropathic pain

Central neuropathic pain (CNP) is a pain syndrome caused by a lesion in the brain or spinal cord that causes great suffering to patients because of the severity and duration of the pain. As a safe and non-invasive neuromodulation technique, transcranial magnetic stimulation (TMS) can exert analgesic effects by activating downstream pain control pathways, inducing neuroplasticity, reducing central sensitization, and regulating neuroinflammatory responses, among other mechanisms. Currently, TMS has been applied to the treatment of central post-stroke pain, central post-spinal cord injury pain, and central post-multiple sclerosis pain, but the analgesic effects obtained vary due to differences in stimulation patterns, treatment periods, and mechanisms of CNP occurrence in different diseases. This paper summarizes the relevant literature on TMS for CNP at home and abroad, and reviews the principle of action, analgesic mechanism, clinical application and efficacy, safety, and future development prospects of TMS.

59 Research status and prospect of gastric cancer risk assessment system

Gastric cancer risk assessment system is an important method for gastric cancer screening and early diagnosis and treatment work to identify high-risk groups of gastric cancer. Such systems include clinical variables and serum markers comprehensive correlation assessment system, clinical variables correlation score system, serum markers correlation score system, gene polymorphism loci related gastric cancer risk prediction system. The results show that the above evaluation system is helpful for gastric cancer screening and early diagnosis and treatment, and has high application value. On the basis of the existing assessment system, experts have found some new risk related variables, which is expected to establish a more efficient and practical assessment system. This article reviews the recent research status and prospects of gastric cancer risk assessment system in gastric cancer screening, aiming to provide reference value for the selection of practical assessment system for gastric cancer screening and early diagnosis and treatment, and provide reference direction for the establishment of a simpler and more efficient assessment system in the future.

63 Research progress of risk assessment models for venous thromboembolism in clinical practice

Venous thromboembolism (VTE) has become the third largest vascular disease in the world, how to screen out the relevant risk factors and identify high-risk peoples is the key to clinical work. There are several VTE risk assessment models (RAM) used in clinical practice. Caprini RAM is mainly used in surgery, Padua prediction score is mainly used in medicine, and Khorana RAM is mainly used in oncology. In different clinical application scenarios, these models still have a few limitations, so designing personalized prediction models has become the main direction of research. This article aims to review the classic VTE risk assessment models and their progress in clinical applications.

68 Application research of “micro lesson” flipped classroom in urology undergraduate practice teaching

Objective To explore application effect of “micro lesson” flipped classroom in urology undergraduate practice teaching. Methods One hundred and forty undergraduate interns of clinical medicine major in 2017 from Air Force Medical University of Chinese People’s Liberation Army were selected as the research objects, and they were divided into traditional teaching group and micro-class flipped teaching group according to random number table method, with 70 students in each group. Traditional teaching group used traditional clinical teaching methods for teaching, and micro-class flipped teaching group used flipped classroom teaching method based on micro-class for teaching. Objective reality and subjective perception indicators of the two groups were compared. The teaching experience of micro-class flipping teaching group was investigated. Results The pre-class preview rate, theoretical examination results, skill application results and case analysis results of micro-class flipped teaching group were higher than those of traditional teaching group, the average time of each class and the time of answering questions and solving questions in each class of micro-class flipped teaching group were shorter than those of traditional teaching group, the analysis and discussion time of each class of micro-class flipped teaching group was longer than that of traditional teaching group, the differences were statistically significant (P<0.05). The classroom satisfaction rate, improve learning efficiency rate, improve learning interest rate, improve self-study ability rate, improve clinical thinking ability rate and application to other courses rate in micro-class flipping teaching group were higher than those in traditional teaching group, and the rate of trying another teaching method in micro-class flipping teaching group was lower than that in traditional teaching group, the differences were statistically significant (P<0.05). The micro-class flipping teaching group reflected three common problems: it took more time and energy to learn before class, it took time to consult the literature and read Chinese Diagnosis and Treatment of Urological Diseases Guide, and the completion rate of interns participate in the pre-class micro-class teaching and discussion was low. Conclusion The flipped classroom teaching method of “micro lesson” can significantly improve the effect of clinical practice teaching for undergraduate urology students. It is a successful attempt in the optimization process of clinical practice teaching, but it still needs continuous exploration, research and improvement, and to make it better serve the clinical practice teaching process.

73 Exploration on the construction path of intelligent medical engineering specialty in traditional Chinese medicine colleges and universities

Intelligent medical engineering is a new major born under the background of the integrated development of new engineering and new medical science. It is an important measure for the reform of higher medical education to adapt to the new stage of medical development. The construction of intelligent medical engineering in colleges and universities that specialty in the traditional Chinese medicine should not only follow the development rules of intelligent medicine, but also integrate the theoretical knowledge of traditional Chinese medicine, aiming to cultivate students’ ability to solve practical problems of traditional Chinese medicine by using artificial intelligence technology, and help the innovation of traditional Chinese medicine. This paper first introduces the development process of intelligent medicine and the present situation of the development intelligent medical engineering, and then analyzes the necessity of establishing intelligent medical engineering in colleges and universities that specialty in traditional Chinese medicine from the needs of economic and social development and discipline construction. Finally, it focuses on the construction path of intelligent medical engineering in Shandong University of Traditional Chinese Medicine.

77 Research on the path to improve the effectiveness of ideological and political course practice teaching in medical colleges

Practical teaching is the key link to realize the soul cultivation of ideological and political courses. In view of the problems existing in the current practical teaching of ideological and political courses, medical colleges and universities should make full use of the unique advantages and combine the characteristics of medical students to build a unique practical teaching path to improve the effectiveness of ideological and political theory courses, that is, to strengthen the construction of practical teaching team of ideological and political courses as a starting point to improve the quality of practical teaching. Build a practical teaching platform by integrating practical teaching resources of ideological and political courses. Basing on the construction requirements of “double first class” and “four new”, modern teaching concepts and methods are fully applied to improve the quality of large-class teaching in analytical chemistry under the background of new era. Course guidance is carried out with “course ideology and politics”. The ideological and political elements of the course are integrated into the teaching of large class of analytical chemistry to enrich the course content and organically integrate “knowledge imparting, value guidance and ability training”. The teaching mode of “lecture in large class and seminar in small class” is systematically adopted by the combination use of pre-class, in-class and after-class discussion throughout the pharmaceutical course learning to strengthen course design and content integration. Diversified internet teaching resources are built and used, multiple modern teaching modes are proposed by using internet resources, and the systematic evaluation of the development of large classes and small discussions is assisted based on modern educational means of “internet+”. The teaching reform and practice of improving the quality of large class course of analytical chemistry, which is the basic course of pharmaceutical major, have been carried out comprehensively and systematically, and the teaching quality of analytical chemistry course and the training level of pharmaceutical talents have been effectively improved.

89 Effect of Lianlian Capsules on Helicobacter pylori related chronic non-atrophic gastritis with syndrome of dampness-heat of spleen and stomach

Objective To observe the clinical effect of Lianlian Capsules on Helicobacter pylori (Hp) related chronic non-atrophic gastritis with syndrome of dampness-heat of spleen and stomach. Methods A total of 78 patients with Hp related chronic non-atrophic gastritis with syndrome of dampness-heat of spleen and stomach admitted to the Department of Spleen and Stomach Diseases of Shanghai Municipal of Tradition Chinese Medicine Affiliated to Shanghai University of Traditional Chinese Medicine from June 2021 to March 2022 were selected and divided into control group and treatment group according to random number table method, with 39 cases in each group. The control group was treated with Bismuth quadruple therapy, and the treatment group was treated with Lianlian Capsules on the basis of the control group. The course of treatment in both groups was 14 days. The Hp eradication rate, clinical efficacy, safety, and tradition Chinese medicine syndrome score and interleukin (IL)-8 before and after treatment were compared between the two groups. Results In the course of treatment, four patients in the treatment group and four patients in the control group withdrew. There was no significant difference in Hp eradication rate between the two groups (P>0.05). The clinical effect of the treatment group was better than that of the control group, and the difference was statistically significant (P<0.05). After treatment, traditional Chinese medicine syndrome score in two groups were lower than before treatment, and the treatment group was lower than the control group, the differences were statistically significant (P<0.05). After treatment, IL-8 level in the treatment group was lower than that in the treatment group before treatment and control group, and the differences were statistically significant (P<0.05). There were no abnormalities in blood routine and liver and kidney function in the two groups during treatment. Conclusion Lianlian Capsules combined with Bismuth quadruple therapy in the treatment of Hp related chronic non-atrophic gastritis with syndrome of dampness-heat of spleen and stomach has a good effect, can improve clinical symptoms, reduce the level of inflammatory factor IL-8, and the drug use is safe.

93 Influence of massage manipulation on anxiety disorder and γ-aminobutyric acid expression level after opioid addiction

Objective To explore the influence of massage manipulation on anxiety disorder and γ-aminobutyric acid expression level after opioid addiction. Methods From July 2020 to January 2021, 74 patients with post-addiction anxiety disorder entered the Gansu Women’s Compulsory Isolation Drug Rehabilitation Center and Lanzhou Compulsory Isolation Drug Rehabilitation Center at the same detoxication stage were selected and divided into routine detoxification group (37 cases) and massage manipulation group (37 cases) by random number table method. The intervention time was three months. The clinical efficacy of two groups was compared. The Hamilton anxiety scale (HAMA) scale score, somatic anxiety factor score, mental anxiety factor score, and γ-aminobutyric acid level were analyzed and compared between the two groups before and after the intervention. Results One case dropped out of the routine detoxification group during the study. The clinical effect of massage manipulation group was better than that of routine detoxification group, and the difference was statistically significant (P<0.05). After treatment, the HAMA total score, somatic anxiety score, and mental anxiety score of the two groups were lower than those before treatment, and the massage manipulation group was lower than the conventional detox group, the differences were statistically significant (P<0.05). After treatment, the level of γ-aminobutyric acid in massage manipulation group was higher than that in massage manipulation group before treatment, and the difference was statistically significant (P<0.05). Conclusion Chinese massage can activate the expression of endogenous neurotransmitter γ-aminobutyric acid in patients with opioid addiction anxiety disorder, relieve their anxiety symptoms and reduce the desire for drugs.

97 Relationship between serum S100β and cerebrospinal fluid P38 protein and cognitive function after craniocerebral trauma

Objective To investigate the relationship between serum soluble protein-100β (S100β) and cerebrospinal fluid P38 mitogen-activated protein kinase (P38) protein and cognitive function after craniocerebral trauma. Methods Seventy-five cases with craniocerebral trauma from February 2016 to February 2021 in the 904th Hospital of Joint Logistics Support Force of Chinese People’s Liberation Army were selected, who were treated surgically, and serum S100β and cerebrospinal fluid P38 protein levels were measured and divided into postoperative cognitive dysfunction (POCD) group (≤26 points) and non-POCD group (>26 points) according to the 15 d postoperative simple intelligence mini-mental state examination (MMSE) score. The general data and S100β and P38 protein levels of the two groups were compared, and logistic regression was used to analyze the influencing factors of POCD, while ROC was used to assess the predictive value of S100β and P38 protein in POCD. Results There were 21 patients with POCD and 54 patients without POCD in 75 cases 15 d after craniocerebral trauma, and the incidence of POCD was 28.00%. The GCS score of the POCD group was lower than that of the non-POCD group, while the percentage of temporal lobe injury and combined hypertension were higher than those of the non-POCD group, with statistically significant differences (P<0.05). The S100β level in the POCD group was higher than that in the non-POCD group, while the P38 protein level was lower than that in the non-POCD group, with statistically significant differences (P<0.05). Logistic regression analysis showed that GCS score, injury site, combined hypertension, S100β, and P38 protein were all risk factors for POCD (OR>1, P<0.05). ROC analysis showed that S100β and P38 proteins predicted POCD with cut-off values of 1.094 μg/L and 0.323, AUC values of 0.852 and 0.839, and AUC values of 0.942 for the combined assay. Conclusion After craniocerebral trauma the changes of S100 β and p38 protein in cerebrospinal fluid are closely related to cognitive function. It is not only the influencing factor of POCD, but also the reference factor to predict its occurrence.

101 Effects of sST2, hs-cTnI and NLR on early myocardial injury in acute organophosphorus pesticide poisoning

Objective To investigate the effects of soluble suppression tumorigenicity 2 (sST2), high-sensitivity troponin I (hs-cTnI) and neutrophil-to-lymphocyte ratio (NLR) on early myocardial injury in acute organophosphorus pesticide poisoning. Methods The clinical data of 166 patients with acute organophosphorus pesticide poisoning admitted to Fuyang Cancer Hospital in Anhui Province from May 2018 to July 2021 were retrospectively analyzed, and within 3 d of admission, the patients were divided into myocardial injury group (32 cases) and non-myocardial injury group (134 cases) according to whether they had concomitant myocardial injury. The levels of sST2, hs-cTnI, and NLR were measured on admission for all study subjects, and logistic regression was used to analyze the influencing factors, and the predictive value of sST2, hs-cTnI, NLR, and the combination of the three for early secondary myocardial injury in acute organophosphorus pesticide poisoning. Results The time from poisoning to admission, amount of poison taken, acute physiological, and chronic health score (APACHE Ⅱ) at admission, white blood cell count, creatine hormone isoenzyme (CK-MB), lactate dehydrogenase, sST2, hs-cTnI, NLR, and proportion of severe poisoning in the myocardial injury group were higher than those in the non-myocardial injury group, and the differences were statistically significant (P<0.05). The area under the curve (AUC) values for sST2, hs-cTnI, NLR, and the combination of the three to predict early secondary myocardial injury in acute organophosphorus pesticide poisoning were 0.718, 0.745, 0.780, and 0.853, respectively. Conclusion sST2, hs-cTnI, and NLR can be used to predict early secondary myocardial injury in acute organophosphorus pesticide poisoning, and the predictive value of the combination of the three is higher.

105 Effect of extracorporeal shock wave therapy combined with respiratory training on chronic primary low back pain

Objective To observe the effect of extracorporeal shock wave therapy combined with respiratory training on chronic primary low back pain (CPLBP). Methods Ninety patients with CPLBP admitted to the First People’s Hospital of Nantong, Jiangsu Province, from March 2020 to March 2021 were selected and divided into extracorporeal shock wave group, respiratory training group and combined group by using random number table method, with 30 cases in each group. All patients received conventional rehabilitation training, to which extracorporeal shock wave therapy was added in the extracorporeal shock wave group, respiratory training was added in the respiratory training group, while extracorporeal shock wave therapy and respiratory training were added in the combined group. Patients in the three groups were evaluated for outcome using the visual analogue score (VAS), Oswestry dysfunction index (ODI), and the Japanese Orthopaedic Association (JOA) lower back pain score before and after four weeks of treatment, respectively, while the recurrence rate of low back pain in the three groups were also observed at follow-up six months after the end of treatment. Results After treatment, the VAS scores of the three groups were lower than before treatment, while that of the combined group was lower than those of the extracorporeal shock wave group and the respiratory training group, and the differences were statistically significant (P<0.05). The ODI of the three groups was lower than before treatment, while that of the combined group was lower than those of the extracorporeal shock wave group and the respiratory training group, and the differences were statistically significant (P<0.05). The JOA scores of the three groups were higher than before treatment, while that of the combined group was higher than those of the extracorporeal shock wave group and the respiratory training group, and the differences were statistically significant (P<0.05). The recurrence rate in the combined group was lower than that in the shock wave and respiratory training groups, and the differences were statistically significant (P<0.05). Conclusion Extracorporeal shock wave therapy combined with respiratory training can effectively relieve pain, improve functional limitation of the lumbar spine, reduce the recurrence rate of low back pain, and improve the quality of survival of patients with CPLBP.

110 Effect of Meropenem de-escalation therapy in the treatment of elderly patients with chronic obstructive pulmonary disease complicated with pneumonia

Objective To explore the effect of Meropenem de-escalation therapy (DET) in the treatment of elderly patients with chronic obstructive pulmonary disease (COPD) complicated with pneumonia. Methods A total of 82 elderly patients with COPD complicated with pneumonia admitted to the 904th Hospital of the Joint Logistics Support Force of Chinese People’s Liberation Army from June 2018 to June 2021 were selected and divided into two groups by random number table method, with 41 cases in each group. The control group was given routine antibiotics, and the observation group was given Meropenem DET. The treatment time of both groups was eight days. The curative effect and the disappearance time of dyspnea, pulmonary moist rales, cough were compared between the two groups. Sputum of the two groups after treatment was collected for bacterial culture, and the bacterial clearance rate was calculated. The level of inflammation before and after treatment in the two groups was detected and the safety of the treatment scheme were observed. Results The clinical efficacy of the observation group was better than that of the control group, and the disappearance time of dyspnea, pulmonary moist rales, cough was shorter than those of the control group (P<0.05). There were no significant differences in bacterial clearance rate and total incidence of adverse reactions between the two groups (P>0.05). After treatment, the levels of C-reactive protein and procalcitonin in two groups were lower than those before treatment, and observation group was lower than control group (P<0.05). Conclusion Meropenem DET can effectively improve the therapeutic effect of elderly patients with COPD complicated with pneumonia, improve the serum inflammation level and clinical symptoms of patients, and is safe and reliable.

114 Meta-analysis on the efficacy and safety of dural puncture epidural block with 25G Whitacre needle for labor analgesia

Objective To assess the efficacy and safety of dural puncture epidural block (DPE) with 25G Whitacre needle for labor analgesia. Methods The PubMed, Embase, The Cochrane Library, Web of Science, CNKI, Wanfang Data, VIP, China Biology Medicine disc were searched by computer, randomized controlled trials (RCT) of DPE with 25G Whitacre needle for labor analgesia were collected from inception to July 2022. Literature screening, data extraction, and quality evaluation were performed, and meta-analysis was performed using RevMan 5.3 software. Results A total of 11 RCT were included, involving 1 678 puerperas, including 839 puerperas in epidural block (EP) group and 839 puerperas in DPE group. Meta-analysis showed that the incidence of asymmetric block (OR=0.31, 95%CI: 0.21-0.47, P<0.000 01), the incidence of analgesia insufficiency (OR=0.49, 95%CI: 0.36-0.67, P<0.000 01), and incidence of motor block with Marcaine (OR=0.38, 95%CI: 0.18-0.81, P=0.01) were lower than those in EP group, and the incidence of bilateral sensory block plane to S2 level (OR=19.61, 95%CI: 8.22-46.78, P<0.000 01) was higher than that in EP group. There were no significant differences in the duration of the first and second stages of labor, the rate of cesarean section, the rate of vaginal instrument assisted delivery, the Apgar score of newborns within 1 min, and the incidence of Apgar score<seven points of newborns within 1 min between the two groups (P>0.05). There were no significant differences in the incidence of hypotension, headache, pruritus, motor block with Ropivacaine in puerperas, and incidence of fetal heart slowdown between the two groups (P>0.05). Conclusion Compared with EP, DPE with 25G Whitacre needle can improve the analgesic effect of labor and does not increase the incidence of maternal and neonatal adverse reactions.

119 Application of Dexmedetomidine in painless gastroenteroscopy in elderly patients

Objective To investigate the application effect of Dexmedetomidine in painless gastroenteroscopy in elderly patients. Methods A total of 340 patients who underwent painless gastroenteroscopy in Fuxin General Hospital of Liaoning Health Industry Group from September 2018 to April 2019 were selected as the research objects, and they were divided into two groups according to random number table method, with 170 patients in each group. The control group was treated with 1.0 mg/kg Propofol intravenously + 0.1 μg/kg Fentanyl, and maintained with 2 mg/(kg·h) Propofol pump during gastroenteroscopy; observation group was additionally injected with Dexmedetomidine for 15 min at 30 μg. The induced dosage of Propofol and recovery time were compared between two groups; the heart rate (HR), mean arterial pressure (MAP), and oxygen saturation (SO2) of two groups were compared before administration (T0), before induction (T1), at the beginning of examination (T2), 10 min after examination (T3), and after examination when Stward score reached 6 points (T4); the incidence of hiccup, bradycardia, tachycardia, intraoperative hypotension, and respiratory depression of two groups were compared. Results At T2-T3, HR, MAP, and SO2 in control group were lower than those at T0, and the differences were statistically significant (P<0.05); at T1-T4, HR, MAP, and SO2 in observation group were higher than those in control group, and the differences were statistically significant (P<0.05). The induced dosage of Propofol in observation group was lower than that in control group, and the recovery time was shorter than that in control group, and the differences were statistically significant (P<0.05). The incidence of hiccup, bradycardia, tachycardia, intraoperative hypotension, and respiratory depression in observation group was lower than that in control group, and the difference was statistically significant (P<0.05). Conclusion Dexmedetomidine in painless gastroenteroscopy patients can reduce the amount of Propofol induction, shorten the recovery time, reduce the impact on hemodynamics, and reduce the incidence of adverse reactions.

123 Effects of Parecoxib Sodium advanced analgesia at different time points on analgesia and narcotic dosage after shoulder arthroscopic rotator cuff repair

Objective To investigate the effects of Parecoxib Sodium on advanced analgesia at different time points on analgesia and narcotic dosage after shoulder arthroscopic rotator cuff repair. Methods A total of 177 patients with shoulder arthroscopic rotator cuff repair who were admitted to Li Huili Hospital, Ningbo Medical Center, Zhejiang Province from April to December 2021 were selected, they were divided into group A (40 mg of Parecoxib Sodium was given 10 min before anesthesia induction, and normal saline was given 10 min before the end of surgery, 59 cases), group B (normal saline was given 10 min before anesthesia induction, and 40 mg of Parecoxib Sodium was given 10 min before the end of surgery, 59 cases), and group C (normal saline was given 10 minutes before anesthesia induction and 10 minutes before the end of surgery, 59 cases). The use of Remifentanil was compared among three groups; visual analogue scale (VAS) scores of three groups were compared 1 h before surgery, and 3 and 6 h after surgery; the number of self-controlled analgesia compressions 24 h after surgery and the amount of Fentanyl were compared among the three groups, and the occurrence of adverse reactions in three groups was recorded. Results The dosage of Remifentanil in group A was lower than that in groups B and C, and the differences were statistically significant (P<0.05). The pairwise comparison of VAS scores at different time points in group A and B showed statistically significant differences (P<0.05). The VAS score of group A was lower than that of group B and C at 3 and 6 h after surgery; VAS score of group B was lower than that of group C, and the differences were statistically significant (P<0.05). The number of self-controlled analgesia compressions and the amount of Fentanyl in group A and B were lower than those in group C 24 h after surgery; the number of self-controlled analgesia compressions 24 h after surgery and the amount of Fentanyl in group A were lower than those in group B, and the differences were statistically significant (P<0.05). There was no significant difference in the total incidence of postoperative adverse reactions among three groups (P>0.05). Conclusion Preanalgesia with Parecoxib Sodium before anesthesia induction can relieve postoperative pain, reduce the number of patient-controlled analgesia compressions 24 h after surgery and the amount of Fentanyl after surgery, and it is safe and reliable.

This article introduces Professor Zhang Huadong’s experience in treating fibromyalgia syndrome with the theory of “hold the center” of Chinese medical master Lu Zhizheng. Professor Zhang Huadong inherits the theory of “hold the center” of Chinese medicine master Lu Zhizheng, and believes that the core disease of this disease lies in the spleen and stomach. This paper deeply analyzes the theory of “hold the center and move the four sides”, emphasizing the importance of the spleen and stomach to produce qi and blood, and to nourish the other four viscera and limbs. Furthermore, the theory of “adjust the emotion” and the mechanism of “hold the center, adjust the rise and fall” are discussed respectively, and the importance of unobstructed qi and blood and happy mood for fibromyalgia syndrome is pointed out. The clinical syndrome type of this disease is more complex, and the clinical syndrome of insufficient qi and blood, stagnation of qi and blood stasis, damp obstruction middle jiao are more common. The treatment emphasizes on strengthening and protecting the spleen and stomach as the core, the selection is flexible, and the clinical effect is quite good.

131 Bibliometric analysis of Guizhi Decoction based on CiteSpace

Objective To explore its research hotspots and frontiers through a visual analysis of related researches on Guizhi Decoction. Methods Core journal literatures related to Guizhi Decoction were retrieved from VIP, Wanfang Data, and China National Knowledge Infrastructure from January 1989 to December 2021, and the time, author, institution, and keywords of the literatures were analyzed using CiteSpace 5.8.R3 software. Results A total of 741 literatures were included, and the number of annual publications of Guizhi Decoction showed an increasing trend. The core research team represented by Jiang Tingliang and Guo Shuying appeared. Research institutions were mainly traditional Chinese medicine colleges, such as Institute of Traditional Chinese Medicine of China Academy of Chinese Medical Sciences, Nanjing University of Chinese Medicine, and so on, but there was less communication among research institutions. Research hotspots of Guizhi Decoction included three aspects of theoretical discussion, clinical application, and experimental research, and there were 18 categories of keyword cluster. The experience of famous doctors was an emergent word lasting from 2011 to now. Conclusion The analysis shows that the reasearch hotspots are that Guizhi Decoction can improve joint disease, regulate gastrointestinal peristalsis, and body temperature, and explore cardiovascular disease and immune system disease through nutrient qi and defensive qi theory.

135 Mesh meta-analysis of the effectiveness of traditional Chinese medicine therapy in the treatment of tension headache

Objective To systematically evaluate the effect of different traditional Chinese medicine therapy on tension headache. Methods The published clinical randomized controlled trials involving traditional Chinese medicine therapy in the treatment of tension headache in Chinese databases (CNKI, VIP, Wanfang Data) and English databases (PubMed, Embase, Web of Science, The Cochrane Library) were searched by computer. The search period was from January 2012 to December 2021. Data extraction and mesh meta-analysis were performed by NoteExpress, Stata 14.2, and RevMan 5.4 software. Results Thirty-two randomized controlled trials involving 3 003 patients were included, involving six different interventions. The results of mesh meta-analysis showed that the total effective rate of Chinese materia medica (RR=0.25, 95%CI: 0.15-0.35, P<0.001), acupuncture (RR=0.27, 95%CI: 0.20-0.55, P=0.03), acupuncture + massage (RR=0.28, 95%CI: 0.19-0.55, P=0.03), acupuncture + scrapping therapy (RR=0.32, 95%CI: 0.17-0.35, P=0.02), acupuncture + Chinese materia medica (RR=0.31, 95%CI: 0.29-0.43, P=0.01) were all higher than that of conventional western medicine. The Begg test suggested a small publication bias (P<0.05). Conclusion Chinese materia medica, acupuncture, acupuncture + massage, acupuncture + scrapping therapy, acupuncture + Chinese materia medica are superior to conventional western medicine, and traditional Chinese medicine therapy may have more advantages in the treatment of tension headache.

The incidence of insomnia in modern society is getting higher. Professor Yao Naili summarizes the experience of predecessors and his own clinical practice, believing that the overall pathogenesis of insomnia is the imbalance of yin and yang, and yang does not enter yin, the pathological factors are mainly attributed to hot, phlegm, blood stasis, and food accumulation. The pathogens block the body, the transportation of yin and yang is blocked, and the patients suffer from insomnia. Professor Yao Naili advocates the “unblocking the road” approach to insomnia. In the treatment of clinical syndrome, clearing heat and removing fire, reducing phlegm and turbidity, promoting blood circulation, and removing blood stasis, eliminating food to dispel pathological factors, smooth the road of yin and yang. This article summarizes professor Yao Naili’s experience in treating insomnia for clinical reference.

Professor Cao Yongqing fully consideres the characteristics of constipation and the pathogenesis of each syndrome type of constipation, so as to form corresponding drug groups, such as strengthening vital qi drug group milkvetch root, largehead atractylodes rhizome, tangshen, heterophylly falsestarwort root strengthening vital qi; nourishing blood clearing intestine drug group chinese angelica, hemp seed, debark peony root, cultivated land; nourishing yin increase fluid drug group dwarf lilyturf tuber, dendrobium, coastal glehnia root, chinese yam; tonifying kidney warming yang drug group cassia bark, desertliving cistanche, twotoothed achyranthes root, malaytea scurfpea fruit; activating qi and removing food stagnation drug group banksian rose, orange fruit, officinal magnolia bark, immature tangerine peel; purgating heat and bowels drug group figwort root, immature orange fruit, officinal magnolia bark, rhubarb root and rhizome; warming interior and dispelling cold drug group rhubarb root and rhizome, prepared common monkshood branched root, zingiber. Professor Cao Yongqing adopts corresponding drug groups for different syndrome types of constipation, which can be flexibly used and combined regardless of the syndrome type. The clinical effect is significant, and his experience is worth promoting.

148 Research on the medication law of traditional Chinese medicine doctor han shirong in the treatment of vitiligo based on data mining platform

Objective To better guide the clinical practice, this paper summarizes Han Shirong’s theoretical ideas and clinical experience in the treatment of vitiligo through the data mining system and looks for the hidden rules. Methods Based on the traditional Chinese medicine inheritance platform system V2.50 software, the outpatient medical records of vitiligo patients diagnosed and treated by Han Shirong, a famous traditional Chinese medicine doctor, from June 2020 to June 2021 was collected. After data screening and processing, the records were entered into the software. The entered prescriptions were analyzed comprehensively by data mining, such as drug use frequency statistics, statistics of four nature of drugs, five flavours, and channel tropism, analysis of prescription rules and association rules, and complex system entropy cluster, etc. Results After screening, 819 prescriptions were included, including 122 kinds of traditional Chinese medicine. In order of frequency, the drugs with high frequency were malaytea scurfpea fruit, fried largehead atractylodes rhizome, indian bread, milkvetch root, fried coix seed, ect.; the medicinal properties were mainly warm and flat, the medicinal tastes were mainly sweet and pungent, and the main ones were spleen, liver and kidney. The commonly used high-frequency drug pairs were “fried largehead atractylodes rhizome-malaytea scurfpea fruit”, “malaytea scurfpea fruit-indian bread” and “fried atractylodes-fried largehead atractylodes rhizome”. Cluster analysis obtained nine new prescriptions, the effect of which was mainly to tonify qi and spleen, clearing heat and dampness, and nourishing liver and kidney. Conclusion Professor Han believes that the core pathogenesis of vitiligo is the disorder of qi and blood caused by internal fire. The treatment focus is on reducing the potential heat between skin surface and muscle. Professor Han takes “Ganwen Chure”, “Yinhuo Guiyuan” as the methods. He uses largehead atractylodes rhizome, milkvetch root, tangshen and other drugs to invigorate and raise the Yang. He also uses prepared common monkshood branched root, zingiber and cassia bark to help the kidney astringe yuanyang. Applying indian bread and coix seed to remove the pathogenic factor from the bladder. It emphasize the combination of reinforcing and reducing. It also stress the principle of removing the pathogenic factor without harming the health.

Suprascapular nerve entrapment syndrome is easily confused with cervical spondylosis and shoulder joint diseases in clinic, which makes the treatment be delayed, thus, resulting in atrophy of supraspinatus and infraspinatus muscles and limited shoulder function in the later stage. Early accurate diagnosis and effective intervention are of great significance for the recovery of shoulder joint function and the avoidance of muscular atrophy. Based on the theory of keeping the tendons diseases in tendons, this article discusses the treatment from “jin”, which provides new ideas for clinical related diagnosis and manipulative treatment. During initial period evil encroach fleshy exterior by using precise massage to release the local tendons and remove the nerve trapped point, so that yang qi can recover. The middle course of the disease is so long that blood stasis and qi stagnation, which need to cooperate with the exercise stretching technique to relax the tendons and collaterals, soft tendons to help movement. While in the late, the meridian is short of qi and blood, making the weakness of tendon and muscle, through self-exercise enhancing healthy qi and increase muscle fullness.

157 Diagnostic value of ultrasonography in central lymph node metastasis of papillary thyroid carcinoma

Objective To explore the diagnostic value of ultrasonography in central lymph node metastasis (CLNM) of papillary thyroid carcinoma (PTC). Methods A retrospective analysis was performed on 113 patients with PTC confirmed by pathology who underwent thyroid cancer surgery in Guangdong Provincial Hospital of Chinese Medicine from November 2018 to July 2019. The maximum diameter, shape, edge, aspect ratio, internal echo, calcification, posterior echo, capsule invasion, blood flow signal, and other ultrasonic characteristics were observed; the diagnostic value of ultrasound in CLNM of PTC patients was analyzed. Results Among 113 patients with PTC, preoperative ultrasound showed CLNM in 37 cases, and pathological results showed CLNM in 52 cases. The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of preoperative ultrasonic diagnosis of CLNM in PTC patients were 61.5%, 91.8%, 77.9%, 86.5%, and 73.7%, respectively. There were statistically significant differences in maximum nodule diameter, microcalcification, and capsule invasion in patients with or without CLNM (P<0.05). Conclusion For patients with maximum diameter of PTC nodule ≥1.5 cm, capsule invasion, and microcalcification, cervical CLNM is prone to occur, and preoperative examination and evaluation of cervical lymph nodes should be paid attention to and standardized.

161 Diagnostic value of MSCT and multimodal MRI in acute phase cerebral hemorrhage

Objective To explore the diagnostic value of multi-slice spiral CT (MSCT) and multimodal MRI in acute phase cerebral hemorrhage. Methods A total of 132 patients with suspected acute phase cerebral hemorrhage admitted to Nantong Second People’s Hospital, Jiangsu Province from June 2019 to December 2020 were selected. All patients underwent MSCT and multimodal MRI. With surgical results as the gold standard, the diagnostic value and consistency of MSCT and multimodal MRI in acute phase cerebral hemorrhage were analyzed. Results The distribution of lesions diagnosed by MSCT was mainly in the basal ganglia, and the shape of the lesions was mainly round; the diffusion weighted imaging (DWI) signal features of multimodal MRI diagnosis lesions were mainly mixed signals, and the DWI hyperintensity edema zone around the lesions. The sensitivity, specificity, positive predictive value, and negative predictive value of MSCT in the diagnosis of acute cerebral hemorrhage were 88.62%, 77.78%, 98.20%, and 33.33%; and the Kappa value of MSCT and surgical results was 0.410 (P<0.05), with general consistency. The sensitivity, specificity, positive predictive value, and negative predictive value of multimodal MRI in the diagnosis of acute cerebral hemorrhage were 95.12%, 88.89%, 99.15%, and 57.14%; the Kappa value of multimodal MRI and surgical results was 0.668 (P<0.05), with general consistency. The sensitivity, specificity, positive predictive value, and negative predictive value of MSCT combined with multi-modal MRI in the diagnosis of acute cerebral hemorrhage were 97.56%, 88.89%, 99.17%, and 72.73%, respectively; the Kappa value of MSCT combined with multi-modal MRI diagnosis and surgical results was 0.866 (P<0.05), showing a good consistency.
Conclusion Both MSCT and multimodal MRI can be used for the diagnosis of acute intracerebral hemorrhage, which can provide reference for clinical diagnosis.

Objective To investigate the value of superb microvascular imaging (SMI) and color Doppler flow imaging (CDFI) combined with Thyroid Imaging Reporting and Data System (TI-RADS) in the diagnosis and differentiation of thyroid nodules. Methods A total of 76 patients with thyroid nodules who came Anqing First People’s Hospital, Anhui Province from February 2021 to August 2022 were selected. Imaging examinations were performed by SMI and CDFI in all patients, and all nodules were graded according to TI-RADS. the diagnostic value of SMI combined with TI-RADS and CDFI combined with TI-RADS in malignant thyroid nodules were analyzed. Results A total of 92 nodules were detected in 76 patients, including 61 benign nodules and 31 malignant nodules. The Kappa value of SMI combined with TI-RADS grading diagnosis and pathological results was 0.901 (P<0.01), showing a good consistency; the Kappa value of CDFI combined with TI-RADS was 0.493 (P<0.01), with general consistency. Conclusion It is feasible to differentiate thyroid nodules by SMI and CDFI combined with TI-RADS.

169 Analysis of the effectiveness of refined operation management of county hospitals based on medical value added

Objective To explore the effectiveness of refined operation management based on medical value added and  to provide a theoretical basis for refined operation management of medical institutions. Methods The operation index data of Qinghe Central Hospital before and after the fine management, such as medical added value, outpatient income ratio, drug ratio, and consumption ratio, were selected as the research objects. The operation results before (from January to December in 2017) and after (from January to December in 2021) the fine management were compared. The development trend of operational indicator data from 2017 to 2021 were observed. Results The percentages of outpatient revenue and drugs were higher after refinement management than before refinement management, and the percentages of consumables and operational indicators were lower after refinement management than before refinement management, and the differences were statistically significant (P<0.05). From 2017 to 2021, the percentage of medical value added and outpatient revenue of the hospital would increase year by year, while the percentage of drugs and consumption would decrease year by year, and the data of each operation index would be continuously optimized. Conclusion The implementation of refined operation management with medical value added as the performance target is effective and can drive the overall development of the hospital, which is worth promoting and applying.

173 Application effect of improved head-low-foot-high lithotomy placement technique in lumpectomy based on comfort care theory

Objective To analyze the effect of improved head-low-foot-high lithotomy placement technique based on comfort care theory in lumpectomy for gynecologic malignancies. Methods One hundred patients with gynecological malignancies admitted to Ningbo Women’s and Children’s Hospital, Zhejiang Province from October 2019 to February 2020 were divided into two groups of 50 cases each using the random number table method. In the control group, the conventional head-low-hip-high lithotomy position was used intraoperatively, while the observation group used a modified head-low-foot-high lithotomy position technique based on comfort care theory. The fluctuations of arterial pressure lthe time points of detection were T1 to T3, i.e. after completion of anesthesia in the flat-lying truncal position [T1], 5 min after pneumoperitoneum with head low, and hip high position [T2], and 30 min after pneumoperitoneum with head low and hip high position [T3], position-related complications, and position comfort were observed. Results The difference in mean arterial pressure levels between the two groups was statistically significant (P<0.05). The mean arterial pressure at T2 and T3 time points in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). At 24 h after surgery, the visual analogue scale(VAS) scores of numbness and pain in the shoulder,and lower limbs of the observation group were lower than those of the control group, and the differences were statistically significant (P<0.05). At 48 h postoperatively, the VAS scores of numbness and pain in the shoulder,and lower limbs were lower in both groups than at 24 h postoperatively, and the differences were statistically significant (P<0.05). At 48 h postoperatively, there were no statistically significant differences in the VAS scores of numbness and pain in the shoulder and lower limbs between the two groups (P>0.05). The proportion of subcutaneous emphysema in the observation group was lower than that in the control group, while the postural comfort score was higher than that in the control group, with statistically significant differences (P<0.05). Conclusion The improved head-low-foot-high lithotomy placement technique based on comfort care theory is used in laparoscopic surgery for gynecologic malignancies, and plays a good role in arterial pressure fluctuation relief, position-related prevention, and pain relief, which could effectively improve the comfort of patients’ position.

177 Effect of balance tank combined with core area training on improving pain degree of patients with lumbar muscle strain

Objective To explore the effect of core area training on improving pain degree of patients with lumbar muscle strain. Methods A total of 106 patients with lumbar muscle strain selected from the Department of Rehabilitation and Acupuncture Changzhou Hospital of Traditional Chinese Medicine of Jiangsu Province from August 2020 to January 2022 were selected as the research object. The patients were divided into control group and observation group, with 53 cases in each group. The control group took balance tank, and the observation group added core area training on the basis of the control group. Visual analog scale score, lumbar Roland-Morris dysfunction questionnaire score, lumbar motion,and quality of life were compared before and after intervention between the two groups. Results After intervention, the visual analog scale score and lumbar Roland-Morris dysfunction questionnaire score of two groups were lower than those before intervention, and the observation group was lower than the control group, and the differences were statistically significant (P<0.05); the lumbar motion of the two groups was higher than that before intervention, and the observation group was higher than the control group, and the differences were statistically significant (P<0.05); the quality of life scores of the two groups were higher than those before intervention, and the observation group was higher than the control group, and the differences were statistically significant (P<0.05). Conclusion For patients with lumbar muscle strain, the combination of balance tank and core area training can effectively reduce the degree of daily pain, improve waist dysfunction, and enhance the range of daily motion of the lumbar spine, which is conducive to enhancing the quality of life.

181 Application effect of fracture liaison service intervention strategy in postoperative nursing of patients with brittle fracture of spinal

Objective To investigate the effect of fracture liaison service (FLS) intervention in postoperative nursing of patients with brittle fracture of spinal. Methods A total of 80 patients with spinal fragility fracture treated in the Department of Orthopedics, Wenzhou People’s Hospital of Zhejiang Province from January to May 2021 were selected as the study objects, according to random number table method, the patients were divided into observation group and control group, with 40 cases in each group. The control group received routine nursing, and the observation group received FLS intervention on the basis of routine nursing. The medication compliance after discharge, the occurrence of falls and secondary fractures after discharge, and the improvement of life quality were observed. Results At six months after discharge, the score of medication compliance in the control group was lower than that at admission, while the score in the observation group was higher than that at admission, and the observation group higher than the control group, and the difference was statistically significant (P<0.05); the incidence of falls and secondary fractures in the observation group were lower than those in the control group within six months after discharge, and the differences were statistically significant (P<0.05); at six months after discharge, the total score of the health survey summary table in both groups was higher than that at admission, and the observation group was higher than the control group, and the difference were statistically significant (P<0.05). Conclusion FLS intervention in the nursing of patients with brittle fracture of spinal can improve patients’ medication compliance after discharge, effectively reduce the risk of postoperative falls and re-fractures, and significantly improve patients’ quality of life.

185 Bibliometric study on nursing of brachial plexus injury in China

Objective Through the bibliometric study of nursing literature of brachial plexus injury in China, the research focus and development direction of this field are clarified. Methods The key words “brachial plexus”, “injury”,and “nursing” were used to search China National Knowledge Network, Wanfang Medical network,and VIP Chinese database, The retrieval period was from the establishment to December 2021. Data mining and mapping were carried out through the database literature visualization analysis system of China National Knowledge Infrastructure, excel, CiteSpace V,and other software. The number of annual publications, published journals, publishing units,and keywords were analyzed statistically and cluster. Results At present, there were only 114 papers related to the nursing of brachial plexus injury in China. Among the top ten journals in terms of the number of published articles, only four were statistical source journals, and the others were general journals; among the top ten medical institutions, there were five hospitals featured with orthopedics, one hospital featured with neurology, one hospital featured with pediatrics, and the rest were comprehensive hospitals. However, there were no secondary or community hospitals in the top ten institutions; in this field, rehabilitation nursing, psychological nursing, functional exercise and functional recovery were the most concerned problems at present. Conclusion At present, the most effective treatment for brachial plexus injury is surgery. Therefore, active rehabilitation and nursing work after surgery are of great significance for the functional recovery and symptom improvement of brachial plexus injury. However, the research field of brachial plexus injury nursing in China is relatively weak and develops slowly. At this stage, the main development direction is rehabilitation nursing and psychological nursing, in addition, relevant medical workers are urged to pay attention to the nursing work of brachial plexus injury, accelerate the research process in this field, so as to provide more effective and scientific data support and ideas for the clinical nursing work of patients with brachial plexus injury.

190 Influence of evidence-based combined with enhanced risk management in operating room of infectious diseases hospital on infection control of perioperative patients

Objective To explore the effect of evidence-based combined with enhanced risk management in the operating room of infectious diseases hospital on perioperative complications and infection control. Methods A total of 129 patients with infectious diseases admitted to the operating room of the Fifth People’s Hospital of Wuxi, Jiangsu Province and the Department of General Surgery the 904th Hospital of the Joint Logistics Support Force  of Chinese People’s Liberation Army from August 2020 to April 2021 were selected. According to the nursing time, the patients were divided into control group (63 cases from August to December 2020) and observation group (66 cases from January to April 2021). The control group received routine nursing, and the observation group received evidence-based risk management. After one month of nursing, the risk awareness, nursing risk events, patient complications, equipment disinfection, and iatrogenic infection of nursing staff were compared between the two groups. Results After intervention, the risk awareness score of nursing staff in the observation group was higher than that before intervention, and the observation group was higher than the control group (P<0.05). The total incidence of risk events in observation group was lower than that in control group (P<0.05). The total incidence of perioperative complications in observation group was lower than that in control group (P<0.05). The qualified rate of postoperative instrument disinfection and cleaning in observation group was higher than that in control group (P<0.05). No iatrogenic infection of nursing staff occurred in the two groups. Conclusion Evidence-based combined with enhanced risk management in the operating room of infectious disease hospital can increase the risk awareness of nursing staff, reduce the occurrence of risk events, and reduce the occurrence of perioperative complications of patients.

194 Effect of pain evidence-based nursing on pain degree, self-management ability, and quality of life in patients with ureteral calculi

Objective To explore the effects of pain evidence-based nursing on pain degree, self-management ability, and quality of life in patients with ureteral calculi. Methods A total of 106 patients with ureteral calculi admitted to North Anhui Coal Power Group General Hospital from February 2020 to February 2022 were selected, they were divided into control group and observation group according to random number table method, with 53 cases in each group. The control group took routine nursing, the observation group took pain evidence-based nursing, both intervention for one month. Pain degree, self-management ability, and quality of life were compared between the two groups before and after intervention. Results After intervention, the visual analog scale scores in both groups were lower than those before intervention, and the observation group was lower than the control group (P<0.05); the scores of self-management ability, and quality of life in the two groups were higher than those before intervention, and the observation group was higher than the control group (P<0.05). Conclusion Evidence-based pain nursing can effectively improve the degree of postoperative pain, self-management ability, and quality of life of patients with ureteral calculi.
