DOI: 10.20047/j.issn1673-7210.2022.32.02
Objective To investigate the role of cytosolic carboxypeptidase 1(CCP1) in the proliferation of mouse testicular primary spermatocytes (GC-2) and its molecular mechanism. Methods GC-2 cells of mouse were cultured in vitro, the cells of negative control group (NC group) and CCP1 overexpression group(GC-2+CCP1 group) were constructed by infecting GC-2 cells with lentivirus containing empty plasmid and CCP1 recombinant plasmid. The influence of CCP1 overexpression on the proliferation and cycle of GC-2 cells were detected by CCK-8 and flow cytometry; the changes of cycle key regulatory proteins in GC-2+CCP1 cells, including cyclin-dependent kinase 2(Cdk2), cyclin B1(Ccnb1) and cyclin D1(Ccnd1) were detected by real-time fluorescence quantitative PCR; the changes of Cdk2 in GC-2+CCP1 cells were analyzed by Western blot. Results CCK-8 and flow cytometry showed that the proliferation ability of GC-2+CCP1 cells in vitro was lower than NC group and cycle of GC-2+CCP1 cells arrest in G2/M phase, the differences were statistically significant (P < 0.05). The mRNA and protein of Cdk2 in GC-2+CCP1 cells was higher than NC group and Cyclin B1 and Cyclin D1 mRNA were lower, the differences were statistically significant (P < 0.05). Conclusion Overexpression of CCP1 can block GC-2 cells in G2/M phase, inhibit the proliferation of GC-2 cells in vitro and affect the expression of Cdk2, Cyclin B1, and Cyclin D1.

DOI: 10.20047/j.issn1673-7210.2022.32.08
programmed death receptor-1 (PD-1) / programmed death ligand-1(PD-L1) immune whistle is a negative immunomodulatory signal, which is activated or suppressed according to the immune status. Targeted drugs based on PD-1 and PD-L1 have been used in tumor immunotherapy at home and abroad and achieved remarkable effects. However, the relevant therapeutic effects of this target in rheumatoid arthritis (RA) are still divergent. Based on the latest research progress, the pathogenesis of RA induced by immune regulation mediated by PD-1/PD-L1 immune whistle is summarized as follows: With the progression of RA and the repeated changes between active and quiescent phases, persistent and/or high expression of PD-1/PD-L1→impaired or depleted immune cell function→disorder of immune tolerance and immune balance→enhanced immune response→promoted or aggravated the progression of RA. This article reviews the role of immune regulation mediated by PD-1/PD-L1 immune whistle in RA.

SHI Min LIU Fulin XIA Xuting LIAO Chenmin
DOI: 10.20047/j.issn1673-7210.2022.32.11
Based on the relationship between intestinal flora and slow transit constipation, this paper discusses the mechanism of traditional Chinese medicine regulating intestinal flora in the treatment of this disease, and believes that the physiological function of intestinal flora is consistent with the holism of traditional medicine and the theory of yin and yang balance. The onset of slow transit constipation is related to intestinal flora imbalance. Traditional Chinese medicine can promote gastrointestinal motility by regulating metabolites of intestinal flora, intestinal neurotransmitters, and intestinal immune response, so as to improve slow transit constipation. This paper reveals the mechanism of traditional Chinese medicine regulating intestinal flora in the treatment of slow transit constipation, and provides theoretical reference for the clinical and scientific research of traditional Chinese medicine in the prevention and treatment of this disease.

Degenerative lumbar spinal stenosis is a disease frequently occurring in the elderly. Conservative treatment of moderate and severe degenerative lumbar spinal stenosis is recommended as the first treatment strategy in the guidelines, but there is no specific high-quality evidence to recommend conservative treatment of Chinese medicine. According to the Chinese medicine theory of “yi jin dai gu” as the treatment principle, a new idea of Chinese medicine comprehensive plan for intervention of moderate and severe lumbar spinal stenosis is explored, that is, a new idea of Chinese medicine conservative treatment plan of “heterozygosity and treatment”. On the basis of the pricking blood treatment and medicine cupping therapy, combined with acupotomology, ointment rubbing, Chinese medicine decoction and other treatment methods as one of the comprehensive therapy. Oral Chinese medicine is used to recuperate the patient’s body and mind, acupotomology is used to increase the intensity of treatment, and ointment rubbing is used to promote tissue repair, forming a four-dimensional integrated therapy of “bamboocuppingas the main therapy, acupotomology as the assist therapy, ointment rubbing as the repair therapy, and Chinese medicine decoction as to improve physical condition”.

DOI: 10.20047/j.issn1673-7210.2022.32.31
In more than 50 years of clinical practice of massage, Professor Wen Qinghan has been able to integrate the western and traditional Chinese medicine. He has drawn on the strengths of various schools, has formed a unique experience and academic ideological system for treating tendon and injuries. Professor Wen strictly followed the traditional principles which emphasize the characteristics of “tendon prefering softness instead of rigidity”, focusing on the inheritance of traditional massage therapy and closely integrating modern medical theory. In the clinical treatment of tendon injuries, Professor Wen emphasizes the application of myofascial chain and muscle starting point therapy, stressing that the practitioner not only needs to be based on the whole body and focus on the local area, but also has to identify the meridian to take the points as well as distinguish the anatomical structure to help who patients restore the mechanical balance of the body and achieve a status of balance between yin and yang. In addition, Professor Wen attaches importance to clinical efficacy, advocates flexible manipulation, opposes excessive massage, and holds the view that practitioners need to continuously strengthen their own manipulations. Professor Wen’s clinical treatment effectiveness is excellent, which deserves learning, refering, inheritance and development.

DOI: 10.20047/j.issn1673-7210.2022.32.37
Objective To explore and analyze the high-risk factors of complications after CT-guided percutaneous lung aspiration biopsy and nursing intervention countermeasures. Methods From May 2018 to October 2021, the clinical data of 196 cases in Tangshan People's Hospital, Hebei Province was retrospectively analyzed. All patients underwent CT-guided percutaneous lung aspiration biopsy for the diagnosis of the disease, and those with postoperative complications were classified as the complication group (42 patients) and the non-complication group (154 patients). The success rate of puncture biopsy and the type of complications in patients were observed, and the high-risk factors affecting complications after CT-guided percutaneous lung puncture biopsy were analyzed. Results The success rate of one-time puncture biopsy in patients was 98.98%, the rate of two punctures was 1.02%, and the complication rate was 21.43%. The differences were statistically significant when comparing the combined COPD, lesion diameter, lesion distance from the chest wall, and puncture needle dwell time between the two groups (P < 0.05). Multifactorial logistic regression analysis showed that combined chronic obstructive pulmonary disease, lesion diameter, lesion distance from the chest wall, and puncture needle dwell time were high risk factors for complications after CT-guided percutaneous lung aspiration biopsy (OR > 1, P < 0.05). Conclusion CT-guided percutaneous lung aspiration biopsy has a high success rate during operation; the high-risk factors of postoperative complications after CT-guided percutaneous lung aspiration biopsy include combined chronic obstructive pulmonary disease, lesion diameter, lesion distance from the chest wall, and puncture needle dwell time, and the analysis of the high-risk factors and the development of effective nursing interventions may reduce the adverse effects of postoperative complications on patients’ health.

DOI: 10.20047/j.issn1673-7210.2022.32.43
Puffer fish poisoning is a relatively rare type of food poisoning. At present, there is no specific antidote for puffer fish poisoning. It is necessary to induce vomiting, gastric lavage, and diuresis as soon as possible to promote the discharge of toxins and symptomatic treatment. This case reports a rare case of recurrent allergic cheilitis due to puffer fish poisoning in a patient without a previous history of food allergy. Professor Lü Wenliang clinically combined with his medical history and pathogenesis, and proposed that the traditional Chinese medicine syndrome differentiation characteristics of tetrodotoxin should be “wind-toxin mainly, and with fire carries dampness”, with “the residual toxin is not clear, the wind toxin is embedded” as the pathogenesis, with “softening liver and dispelling pathogenic wind” as the core treatment method, taking into account “clearing heat and removing toxicity, invigorating spleen for eliminating dampness”, and using traditional Chinese medicine to treat allergic cheilitis secondary to tetrodotoxin poisoning. This case is relatively rare and has remarkable curative effect. It is hereby reported for the benefit of colleagues, aiming to supplement and enlighten the traditional Chinese medicine treatment of allergic cheilitis secondary to puffer fish poisoning.

Thymic malignant melanoma is a rare ectopic thymic tumor with high invasivity. For patients with advanced thymic tumor that cannot be surgically resected, radiotherapy and chemotherapy are the traditional treatment methods, but the effect is poor. Recently, immune checkpoint inhibitors have shown good anti-tumor effects. According to previous reports, the use of immune checkpoint inhibitors in the treatment of thymic tumors and thymic carcinoma causes a high proportion of serious immune-related adverse reactions. There has been no report of immune checkpoint inhibitors in the treatment of thymic malignant melanoma. This case on October 4, 2021 in the Xuzhou Central Hospital, Jiangsu Province, by biopsy and gene detects the definite diagnosis of primary melanoma of the thymus gland malignant, after six cycles of Tririplizumab treatment, chest tightness and wheezing symptoms were relieved, and the tumor was reduced, the treatment effect was evaluated as partial remission, and the patient is still under treatment and follow-up. This article through to the patient’s diagnosis, treatment and follow-up process, review and summarize the related literatures, the thymus gland malignant melanoma is relatively rare, need to identify with other thymic tumor pathologically, using immune checkpoint inhibitors in thymus treatment in patients with malignant melanoma may show a good effect, immune related adverse reaction probability than other thymic tumor is low, it can provide treatment ideas for similar cases in the future.

DOI: 10.20047/j.issn1673-7210.2022.32.45
Human genetic resources are the key resources to carry out life science research. It is very important to standardize the management of human genetic resources in clinical research. Based on the self-examination of human genetic resources management and the accumulation of daily work experience, the Affiliated Hospital of Guizhou Medical University found the following risk points of human genetic resources management in clinical studies: lack of awareness of sponsors/researchers in reporting and unclear types of reporting; the estimate of the number of biological samples required for the test is unreasonable, the collection purpose is not specific, and the supervision of the use is not comprehensive; the rationality of clinical research intellectual property ownership is not in-depth, and the fairness of the distribution of experimental results needs to be enhanced. Based on the above risks point analysis, strengthen the education of the human genetic resources, enhance the consciousness of the declaration, improve the level of scrutiny, clear responsibilities, strengthen the daily supervision, from the four stage of clinical research project initation, start, development and completion, establish and improve the human genetic resources to further improve human genetic resources management system.
