4 The mechanism of LOX promoting gastric cancer metastasis under long-term hypoxic conditions

Objective To investigate the mechanism of lysyl oxidase (LOX) on metastasis promotion in hypoxia-resistant gastric cancer (HRGC) cells. Methods The gastric cancer cell lines, MGC803 and BGC823 were routinely cultured. HRGC cell lines, MGC803/hypo and BGC823/hypo were cultured under 2%O2 conditions. The migration ability of gastric cancer cells was detected by Transwell assay. LOX mRNA level was detected by PCR; LOX siRNA was used for LOX transiently silencing; Western blot was used to detect the protein level of LOX, AKT and the activation of AKT pathways. Results Compared with the parental gastric cancer cells, the migration ability of HRGC cells was significantly enhanced (P < 0.05), the expression level of mRNA and protein of LOX were significantly increased (P < 0.05). After transient silencing of LOX, the migration ability was significantly decreased in HRGC cells (P < 0.05). Compared with parental gastric cancer cells, AKT phosphorylation was significantly increased (P < 0.05), whereas AKT phosphorylation was significantly decreased after transient silencing of LOX in HRGC cells (P < 0.05). Conclusion Long-term hypoxic conditions increase LOX and promote gastric cancer cell metastasis by activating the AKT pathway.

9 Packaging and identification of ubiquitinated HBcAg fusion gene and Tapasin gene expression recombinant lentivirus vectors

Objective To construct and package a recombinant lentivirus stably expressing the ubiquitinated HBcAg fusion gene and the Tapasin gene and to lay the foundation for the experimental study of immunotherapy for chronic hepatitis B. Methods The Ub-HBcAg gene was amplified with designing primers by polymerase chain reaction (PCR). The purified Ub-HBcAg fragment was cloned into the lentivirus vector pLenti-CMV-HA-3Flag-P2A-EGFP which was ligated by restriction endonuclease. The product pLenti-CMV-Ub-HBcAg-HA-3Flag-P2A-EGFP was transformed into E. coli DH5α cells. The positive clones were chosen for PCR identification and direct sequencing analysis. Then, T2A-Tapasin sequence was chemically synthesized and cloned into pLenti-CMV-Ub-HBcAg-HA-3Flag-P2A-EGFP. The product recombinant pLenti-CMV-Ub-HBcAg-HA-T2A-Tapasin-3Flag-P2A-EGFP plasmid was confirmed by direct gene sequencing. Lentiviral particles of LV-Ub-HBcAg/Tapasin were produced by triple transfection of 293T cells with pLenti-CMV-Ub-HBcAg-HA-T2A-Tapasin-3Flag-P2A-EGFP using packaging plasmids pLP1, pLP2 and envelope plasmid pLP/VSVG. The recombinant lentiviral particles were transfected into human embryonic kidney 293T cells. The titer of lentivirus was detected by real-time fluorescent quantitative PCR and the expression of the target gene was determined by Western blot. Results Recombinant lentivirus vectors expressing ubiquitinated HBcAg fusion gene and Tapasin gene were successfully constructed and packaged. The titer was 3.25×108 TU/mL detected by real-time fluorescent quantitative PCR. Conclusion The stable expressing of ubiquitinated HBcAg fusion gene and Tapasin gene recombinant lentivirus vectors are successfully established, providing experimental basis for immunotherapy of chronic hepatitis B.

13 Effect and mechanism of Allicin inhibition on human papillary thyroid carcinoma TPC-1 cells

Objective To investigate the effect and mechanism of Allicin inhibition on the growth of human thyroid papillary carcinoma TPC-1 cells. Methods The cultured TPC-1 cells were divided into the control group and the Allicin treatment groups with different concentrations (10, 20, 30, 40, 50 μg/mL). Methylthiazoletrazolium (MTT) method was used to determine the inhibitory effect of different concentrations of Allicin treatment on the growth of TPC-1 cells at 24, 48, 72 h. Inverted microscope and scanning electron microscope were used to observe the change of cell morphology and ultrastructure after treatment by Allicin. Flow cytometry was used to test the effects of Allicin on cell cycle. Immunocytochemical technique was used to detect the effects on expression of intracellular apoptotic protein Bax and anti-apoptotic protein Bcl-2 after treatment by Allicin. Results Compared with the control group, different concentrations of Allicin could inhibit the growth of TPC-1 cells, the differences were statistically significant (P < 0.05); the inhibition rate of Allicin on cells was related to time and concentration. With the increase of Allicin concentration, the proportion of G2/M phase cells was significantly increased, while that of G0/G1 phase cells was decreased. All pairwise comparison of adjacent concentration groups showed statistically significant differences (P < 0.05) except the control group and 10 μg/mL Allicin group (P > 0.05). Compared with the control group, the intracellular Bcl-2 expression was decreased and Bax expression was increased in every concentration of Allicin treatment group, the differences were statistically significant (P < 0.05); the expression levels of Bcl-2 and Bax in Allicin treatment groups with different adjacent concentrations were compared, the differences were statistically significant (P < 0.05). Conclusion Allicin can inhibit the growth of TPC-1 cells of papillary thyroid cancer, and its mechanism may be related to inhibition of cell proliferation by Allicin and the induction of apoptosis by down-regulating Bcl-2 and up-regulating Bax.

18 Effects of ginseng on intestinal mucosa and the levels of serum CRP, TNF-α and IL-6 in mice after infection

Objective To investigate the effects of ginseng on intestinal mucosa and the levels of serum C-reactive protein (CRP), tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) in mice after infection. Methods Among 60 mice, 15 mice were randomly selected as blank control group, and the remaining 45 mice were used to prepare severe abdominal infection model (infected group). Then 45 mice models were divided into three groups by the random number table method: model group, anti-inflammatory group, anti-inflammatory + ginseng group, with 15 mice in each group. The anti-inflammatory group was given Imipenem/Cilastatin 333.33 mg/kg (intraperitoneal injection, once 12 h); the anti-inflammatory + ginseng group was given intragastric administration of ginsenoside Rb1 (purity>98%) 50 mg/(kg·d) on the basis of the anti-inflammatory group; the model group was given the same amount of sterile saline by intraperitoneal injection. Blood samples were taken at 24, 48 h and 72 h after treatment. Five rats in each group were used to observe the changes of CRP, TNF-α and IL-6 levels in the intestinal mucosa of mice in each group; the terminal ileum was harvested and HE staining was performed. The intestinal mucosal layer was observed and the intestinal epithelial cell apoptosis index was calculated. Results After severe intraperitoneal infection, mice intestinal mucosal atrophied, intestinal epithelial cell apoptosis increased significantly, and the more obvious the intestinal mucosal atrophied, the greater the number of intestinal mucosal epithelial cell apoptosis was. After treatment for 72 h, the levels of CRP, TNF-α and IL-6 in mice with severe intraperitoneal infection of anti-inflammatory + ginseng group were significantly decreased (P < 0.05 or P < 0.01), and the decrease range was greater than that of anti-inflammatory group (P < 0.05); the apoptotic index of anti-inflammatory + ginseng group was significantly lower than that of the model group and the anti-inflammatory group (P < 0.05 or P < 0.01). Conclusion Severe abdominal infection can cause intestinal mucosal atrophy, intestinal mucosal atrophy is related to the number of intestinal epithelial cell apoptosis; ginseng has a protective effect on the intestinal mucosa, which can improve the symptoms of intestinal mucosal atrophy, reduce the impact on intestinal mucosal secretions.

22 Study of the effects and mechanisms of gastrodin to promote glucose consumption in L6 myotubes

Objective To investigate the activities and possible mechanisms of gastrodin (GSTD) to promote glucose consumption in L6 myotubes. Methods Rat L6 skeletal muscle cells were cultured in vitro and differentiated into myotubes. After serum starvation, GSTD was used to treat the myotubes for 24 h, either alone or in combination with insulin at a concentration of 0.05 nmol/L. Metformin (Met) was used a positive control drug. In some experiments, L6 myotubes were treated with 100 nmol/L of insulin for 24 h to induce insulin resistance before drug treatment. Commercial kits were used to determine cell glucose consumption, L-lactate release, and ATP production. The expression levels of target proteins and genes were determined by Western blot and real-time quantitative reverse transcription-PCR (RT-PCR). Results GSTD promoted the basal glucose consumption of L6 myotubes in a concentration-dependent manner [comparing with control cells, the diferences were statistically significant (P < 0.05 or P < 0.01)]. In addition, GSTD greatly increased the insulin-stimulated glucose consumption [comparing with cells treated with insulin or GSTD alone, the diferences were statistically significant (P < 0.05)]. In a state of insulin resistance, GSTD at a concentration of 500 μmol/L was still effective to promote cell glucose consumption (P < 0.05). After treatment, Met significantly increased cellular L-lactate release but decreased ATP production [comparing with control cells, the diferences were highly statistically significant (P < 0.01)]. However, GSTD did not have such effects. GSTD greatly up-regulated the protein and mRNA expression levels of GLUT4 in L6 myotubes. In addition, it activated the AMPK and Akt pathways, as indicated by the significant increase of p-AMPKα and p-Akt levels after treatment [comparing with control cells, the diferences were statistically significant (P < 0.05 or P < 0.01)]. Conclusion When used to treat L6 myotubes, GSTD greatly increases glucose consumption and is able to overcome insulin resistance. The mechanisms may be associated with the up-regulation of GLUT4 as well as the activation of AMPK and Akt pathways.

26 Study of antibacterial of Feiyang Changweiyan Capsules and its main components in vitro

Objective To explore the in vitro antimicrobial activity of Feiyang Changweiyan Capsules and its main components against common bacteria, and to explore the sensitive strains and main antimicrobial components of Feiyang Changweiyan Capsules. Methods Minimum inhibitory concentration (MIC) of Feiyang Changweiyan Capsules, drug-deficient drugs, gallic acid, ellagic acid and syringin were determined by multiple dilution method. Results Feiyang Changweiyan Capsules had the most significant inhibitory effect on Staphylococcus aureus with MIC value of 1.6 mg/mL, it had significant inhibitory effect on Shigella flexneri and Salmonella enteritidis with MIC value of 3.1 mg/mL, and it had significant inhibitory effect on Escherichia coli, Pseudomonas aeruginosa, Escherichia coli (enterotoxin producing strain), Salmonella typhimurium and Salmonella typhimurium with MIC value of 6.3 mg/mL. The MIC value was 50.0 mg/mL for Klebsiella pneumoniae, almost no inhibition for Candida albicans, MIC value was more than 100.0 mg/mL; Feiyang Changweiyan Capsules showed strong inhibition for clinical bacteria including Escherichia coli, Salmonella typhi, Salmonella enteritidis, Shigella flexneri and Proteus vulgaris, MIC values were 6.3, 6.3, 3.1, 3.1 and 12.5 mg/mL, respectively. Conclusion Feiyang Changweiyan Capsules and its main components, gallic acid, ellagic acid and syringin, have strong antibacterial activity. The sensitive strains are Staphylococcus aureus, Shigella flexneri, Salmonella enteritidis, Escherichia coli, Pseudomonas aeruginosa, Escherichia coli (enterotoxin-producing strains), Salmonella typhimurium, Salmonella typhimurium and Proteus vulgaris. Euphorbia hirta L., Polygonum chinense L. and Ilex rotunda thunb. all have certain antibacterial activity, among which the antimicrobial effect of Ilex rotunda thunb. is slightly better than that of other two traditional Chinese medicines, and the main component of the antibacterial activity of Ilex rotunda thunb may be syringin.

30 Problems and improvement research status of black plaster preparation process

Black plaster is one of the traditional dosage forms in ancient China. At present, most of the production of plasters still adopts traditional methods, and there are many drawbacks in the preparation process, such as incomplete extraction of explosives, environmental pollution, fire poisoning problems and so on. In recent years, preparation workers have made effective improvements and changes in the preparation of black plaster, such as optimizing extraction methods, improving plaster matrix, and improving automated production efficiency. On the basis of summarizing and analyzing the problems existing in the preparation of black plaster, this paper sorts out and discusses the current situation of the research on the improvement of the preparation process of black plaster, and looks forward to the research trend of the preparation of black plaster, with a view to promote the reform and development of the preparation of black plaster.

34 Preparation of adriamycin propanediol liposomes and its anti-tumor in vitro

Objective To prepare a novel nano-alcoholic plastid-propylene glycol liposome, namely, adriamycin propanediol liposomes (A-PL) using adriamycin as a model drug and to investigate its anti-tumor effect in vitro. Methods A-PL glycol liposomes were prepared and their microscopic morphology, particle size and potential were observed by transmission electron microscopy, laser particle size analyzer and potentiometer. Human hepatoma cell line HepG-2 was used as the model cell line. The cytotoxicity of A-PL was examined by MTT assay. The uptake of A-PL by HepG-2 was observed by flow cytometry and inverted fluorescence microscopy. The function of P-glycoprotein (P-gp) was detected by rhodamine reagent. Results A-PL had a round appearance, good dispersion and no aggregation. The average particle size was 150 nm, the Zeta potential was -5.78 mV, and the encapsulation rate was 83.7%. The cytotoxicity of A-PL was significantly higher than that of adriamycin solution; A-PL could increase cell uptake of drugs. In addition, A-PL was easier to enter cells and had a strong affinity with the nucleus. A-PL could inhibit the overexpression of P-gp. Conclusion The A-PL prepared in this study overcomes the shortcomings of traditional liposome aggregation, has the advantages of stability and small particle size, and has good anti-tumor effect. The mechanism of action is mainly dependent on the strong endocytosis of A-PL and the inhibition of poloxamer on drug efflux pump.

39 Study on HPLC fingerprint of Mailuotong Granules

Objective To establish a high performance liquid chromatography (HPLC) fingerprint of Mailuotong Granules, and qualitatively analyze its chemical composition by triple quadrupole liquid chromatography-mass (LC-MS) system, and provide evidence for the quality evaluation of Mailuotong Granules. Methods Phenomenex Luna C18 (2) column (4.6 mm × 250 mm, 5 μm) was used. Acetonitrile-0.3% formic acid solution was used as mobile phase for gradient elution. The flow rate was 1.0 mL/min, column temperature was 25℃, and the detection wavelength was 270 nm. Ten batches of Mailuotong Granules were determined. The fingerprint of Mailuotong Granules was established with Similarity Evaluation System for Chromatographic Fingerprint of Traditional Chinese Medicine (2004A edition). The common peaks were analyzed by positive and negative ion modes, and confirmed by the existing reference substances. Results The HPLC fingerprints of Mailuotong Granules were established. A total of 21 common peaks were identified and assigned to 8 herbs. The LC/MS system identified 19 common peaks, 7 of which were confirmed by comparison. Conclusion The method has good reproducibility and strong characteristics, which can provide a basis for the quality control of Mailuotong Granules and provide reference for the material basic research of other traditional Chinese medicine compounds.

43 Study on the preparation process of ginkgolide K

Objective To optimize the preparation process of ginkgolide K. Methods Taking ginkgolide B as raw material, the yield of ginkgolide B as index, the ratio of feed (ginkgolide ∶ diethylaminotrifluoride), the ratio of reaction solvent (ginkgolide ∶ dichloromethane), reaction time and reaction temperature were investigated. The optimum preparation conditions of ginkgolide K were optimized by orthogonal design, and its structure was identified by spectroscopy and the content was detected by high performance liquid chromatography. The high performance liquid chromatography was performed on a ZORBAX SB-C18 chromatography column (250 mm × 4.6 mm, 5 μm) and with the detection wavelength at 220 nm. The mobile phase was water-acetonitrile-isopropyl alcohol (60∶35∶5), flow rate was 1.0 mL/min, column temperature was 30℃. Results The optimum preparation conditions of ginkgolide K were as follows: the feed ratio was 1∶4.5, solvent ratio was 1∶30, reaction time was 20 min and reaction temperature was 4℃. The target compound was identified as ginkgolide K by Q-TOF-MS, 1H-NMR, 13C-NMR and two-dimensional NMR spectroscopy. Conclusion The preparation process is mild, the preparation cycle is short, the operation is simple, the yield is high, and it is suitable for industrial production.

47 Research progress of traditional Chinese medicine in regulating the MAPK signaling pathway of cerebral ischemia

Cerebral ischemia is often associated with various neurological diseases such as ischemic stroke, Alzheimer disease and vascular dementia. The mechanism of pathological injury after cerebral ischemia is very complicated. Long-term cerebral ischemia can cause progressive neurological dysfunction and cognitive dysfunction. At present, there are limited kinds of drugs for cerebral ischemia, and most of them are still in the experimental stage. MAPK is an important signal pathway in eukaryotic cells, which is regulated by many factors and widely expressed in the central nervous system. Studies have shown that ERK1/2, JNk, p38 of Mark family interacting with the signal pathways like caspase-3, Akt, Nrf2, NF-κB and others are involved in signal transduction of early pathological and physiological mechanisms of cerebral ischemia. Combining these signaling pathways with researches of its related gene and protein, this paper reviews the protective mechanism of Chinese medicine on cerebral ischemic neurons in order to provide a theoretical basis for the clinical treatment of cerebral ischemia and lay the foundation for the development of traditional Chinese medicine which can play the role of brain protection through MAPK.

51 Research progress on regulation of Wnt signaling pathway in cardiovascular diseases

In recent years, more and more attention has been paid to the study of the Wnt signaling pathway and cardiovascular diseases. The Wnt signaling pathway regulates the development of the heart and blood vessels, and the pathway will be reactivated in the case of cardiovascular diseases and thus play a regulatory role. This review mainly introduces the main classification of Wnt signaling pathway and its main members, and how members are involved in the occurrence and development of cardiovascular diseases. This review also provides a brief introduction to many potential targets for the regulation of cardiovascular diseases by Wnt pathway and the prospects for the prevention and treatment of cardiovascular diseases by these targets.

55 Research progress on improving myocardial sarcoplasmic reticulum calcium ATPase 2a with the treatment of traditional Chinese medicine in chronic heart failure

Chronic heart failure (CHF ) is the end stage of a variety of cardiovascular diseases, with high incidence, high mortality and expensive treatment. The abnormal expression and activity of cardiac myocyte sarcoplasmic reticulum calcium ATPase 2a (SERCA2a) are the key pathological mechanism for the occurrence and development of CHF. The results of the clinical trial that AAV1/SERCA2a genetically treats CHF were not satisfactory, and the researchers restated that full attention should be paid to drug screening, research and development to improve the endogenous myocardial SERCA2a expression level and activity. Traditional Chinese medicine has a long history in treating CHF with remarkable therapeutic effects. In traditional Chinese medicine, CHF can be classified to the cotogories of "palpitation", "asthma syndrome" and "edema". A lot of researches indicate that Chinese medicine compound, single Chinese herb and Chinese medicine monomer can prevent and treat CHF through improving endogenous myocardial SERCA2a expression and activity.

This paper reviews the origin and development of immune nutrition, and summarizes the unique anti-tumor characteristics of four common clinical immune nutrients, glutamine, arginine, polyunsaturated fatty acids and microecological immune nutrition in digestive tract tumors. As well as the present immunological nutrition in the clinical use process faces the challenge and to the future prospect. Combined with the literatures of immune nutrition at home and abroad in recent years, this paper provides a reference for the clinical nutrition treatment of patients with digestive tract tumors.

Doxofylline is a methylxanthine derivative that acts as a new bronchodilator with good pharmacological effects and clinical efficacy in the treatment of chronic obstructive pulmonary disease and asthma. Compared with traditional theophylline, doxofylline has the characteristics of not antagonizing calcium channel receptors, nor affecting calcium influx, and has a weak affinity for adenosine A1 and A2 receptors, so it does not affect heart rate and rhythm, sleep rhythm, central nervous system (CNS) function and gastric secretion, and has good safety and tolerance. This article reviews the research progress of doxofylline in order to provide reference for the majority of medical workers.

Wnt signaling pathway is a highly conserved signaling pathway, which plays a fundamental role in various human development processes and in maintaining adult tissue homeostasis. The classical Wnt signaling pathway is closely related to the development and differentiation of cells. Wnt signaling pathway plays an important role in maintaining sperm morphology and function in male mammals. The abnormal Wnt signaling pathway is related to male infertility and testicular cancer. This article reviews the latest progress in the role of Wnt signaling pathway in male reproductive system and its relationship with male infertility, in order to further understand the mechanism of Wnt signaling pathway and provide reference for the treatment of male infertility.

75 Application of flipped classroom combined with microlecture in medical undergraduate internship education

Internship education is an important stage of medical undergraduate education connecting medical theory and practice teaching. The traditional practice teaching mode needs to be optimized and innovated by combining network communication and multimedia technology to ensure the effectiveness of teaching. Flipped classroom and microlecture are new teaching model that have been found to be effective on improving teaching outcomes. In our teaching practice, this model was found to help stimulating interest, promoting interaction, highlighting medical humanities, and thus improving the effect of practice teaching. This medical undergraduate internship education model deserves to be recommended and improved if it is carefully designed with the principle of “targeting teaching objectives, planning teaching content reasonably and emphasizing critical thinking”, reasonably implements the form of flipped classroom and designs the content of microlecture.

83 Relationship between neutrophils to lymphocytes ratio and type 2 diabetic kidney disease

Objective To investigate the relationship between neutrophil-to-lymphocyte ratio (NLR) and type 2 diabetic kidney disease (DKD). Methods A total of 148 patients with type 2 diabetes mellitus admitted to Beijing Luhe Hospital from July 2012 to June 2018, and they were divided into 5 groups according to urine albumer excretion rate in 8 h (8°UAER). Thirty patients with 8°UAER<20 μg/min and normal serum creatinine were DM1 group; 30 patients with 20 μg/min≤8°UAER<200 μg/min and normal serum creatinine were DM2 group; 28 patients with 8°UAER≥200 μg/min and normal serum creatinine were DM3 group; 29 patients with 8°UAER≥200 μg/min and serum creatinine>120 μmol/L, not yet been dialysis were DM4 group; 31 patients diagnosed as DKD and been dialysis were DM5 group. Twenty-nine health control individuals were selected as control group (N group). Blood was collected by fasting vein. Blood routine examination, glucose, lipid, liver and kidney function were examined in each group. Eight-hour urine in DM1, DM2, DM3 and DM4 group were collected and 8°UAER were detected. Results NLR gradually increased in DKD progress and NLR in DM4 and DM5 group were higher than that in N, DM1, DM2, DM3 group (P < 0.05 or P < 0.01), NLR in DM3 group was higher than that in N and DM1 group (P < 0.05). The results of Pearson correlation analysis showed that NLR was positively correlated with age (r = 0.317, P < 0.001), course of disease (r = 0.306, P < 0.001), 8°UAER (r = 0.293, P = 0.006), urea nitrogen (r = 0.404, P < 0.001), total parathyroid hormone (iPTH) (r = 0.465, P < 0.001) and was negatively correlated with glomerular filtration rate (r = -0.438, P < 0.001), albumin (r = -0.194, P = 0.019), high-density lipoprotein cholesterol (r = -0.182, P = 0.028). Multiple linear regression equation: YLogNLR = 0.29+0.54Log8°UAER+0.004 age (R2 = 0.152, P = 0.001). ROC curve showed NLR=2.29 as the cut point, and the sensitivity and specificity of DKD diagnosis were 52.6% and 73.3% (area under the ROC curve is 0.661, 95%CI: 0.544-0.779, P = 0.014). Conclusion NLR gradually increases during the progression of diabetic nephropathy, and is significantly high in patients with type 2 diabetes mellitus with proteinuria. It is positively correlated with 8°UAER, but the increase of NLR is later than 8°UAER, suggesting that increasing NLR may predict the occurrence of diabetic kidney disease.

88 The effect of surgical treatment for pancreatitis and its influence on the level of TNF-α and IL-6 in patients

Objective To investigate the effect of surgical treatment for pancreatitis and its effect on the levels of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6). Methods Fifty patients with pancreatitis treated in Department of General Surgery, Jiangsu Province Hospital of Chinese Medicine, affiliated to Nanjing University of Chinese Medicine from August 2012 to January 2016 were selected. All the patients underwent surgical treatment. According to the type of operation, they were divided into control group (25 cases) and observation group (25 cases). The control group was treated with laparotomy, the observation group was treated with laparoscopic surgery. Treatment effect was evaluated on the 5th day after operation. The operative time, intraoperative blood loss, total surgical cost, blood amylase recovery time and intestinal function recovery time were recorded and counted. The incidence of incision infection, lung infection, vasculitis, leukopenia in the two groups were recorded and counted. The effect of surgery on the levels of TNF-α, IL-6 were compared. Results The operative time, intraoperative blood loss, total surgery cost, blood amylase recovery time and recovery time of intestinal function in the observation group were all shorter/less than those in the control group (P < 0.05). The levels of TNF-α and IL-6 5 d after operation in the two groups were lower than those before treatment (P < 0.05), and the levels of TNF-α and IL-6 in the observation group were lower than those in the control group (P < 0.05). The incidence of incision infection, lung infection, vasculitis and leucopenia rate in the observation group 5 d after operation were lower than those in the control group (P < 0.05). Conclusion The use of laparoscopic surgery in treating patients with pancreatitis can shorten the time for symptom improvement. The surgical trauma is small, and it can reduce the level of postoperative inflammatory factors and the incidence of postoperative complications. It is worthy of popularization and application.

92 Effect of continuous plasma filtration adsorption in the treatment of severe acute pancreatitis

Objective To investigate the effect of continuous plasma filtration adsorption (CPFA) on severe acute pancreatitis and its effects on tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and interleukin-10 (IL-10). Methods A total of 40 patients with severe acute pancreatitis were selected from October 2014 to June 2018 in the Department of Severe Medicine of Dalian Central Hospital. They were divided into control group (n = 20) and treatment group (n = 20) according to the treatment method. The control group was treated with continuous venovenous hemofiltration (CVVH) for 72 hours on the basis of routine treatment, while the treatment group was treated with CPFA on the basis of the control group. The levels of TNF-α, IL-6 and IL-10 in blood were measured before and after treatment for 3 and 7 days. The vital signs, intra-abdominal pressure, acute physiology and chronic health (APACHEⅡ) scores were compared between the two groups before and after treatment, and the organ function was evaluated, adverse reactions and prognosis were observed. Results There was no significant difference in the clinical indexes between the two groups before treatment (P > 0.05). After 3 days of treatment, the heart rate, oxygenation index, intra-abdominal pressure and APACHE Ⅱ scores of the two groups were lower than those before treatment, and the average arterial pressure and white blood cell count were higher than those before treatment (P < 0.05); the heart rate and intra-abdominal pressure of the treatment group were lower than those of the control group, and the average arterial pressure was higher than that of the control group (P < 0.05). After 7 days of treatment, the heart rate, white blood cell count, intra-abdominal pressure and APACHE Ⅱ scores of the two groups were lower than those before treatment, and the average arterial pressure and oxygenation index were higher than those before treatment (P < 0.05); the heart rate, intra-abdominal pressure and APACHE Ⅱ scores of the treatment group were lower than those of the control group, and the average arterial pressure was higher than that of the control group, with significant differences (P < 0.05). There was no significant difference in the levels of TNF-α, IL-6 and IL-10 between the two groups before treatment (P > 0.05). After 3 days of treatment, the levels of pro-inflammatory cytokines TNF-α, IL-6 in the two groups decreased, the levels of TNF-α and IL-6 in the treatment group were lower than those in the control group (P < 0.05), and the levels of anti-inflammatory cytokines IL-10 in both groups were increased (P < 0.05), and the level of IL-10 in the treatment group was higher than that in the control group (P < 0.05). After 7 days of treatment, the levels of pro-inflammatory cytokines in the two groups decreased further, and the treatment group was lower than the control group, the difference was statistically significant (P < 0.05); while the anti-inflammatory cytokine levels were further increased, and the treatment group was higher than the control group, the difference was statistically significant (P < 0.05). The average hospitalization days and the time needed to continue blood purification treatment in the treatment group were significantly shorter than those in the control group (P < 0.05), but there was no significant difference in mortality between the two groups (P > 0.05). Conclusion CPFA can better regulate the levels of cytokines in the blood, thereby improving clinical symptoms, shortening treatment time and treating severe acute pancreatitis.

96 Influencing factors of successful extubation and withdrawal in patients with primary cerebral hemorrhage and respiratory failure

Objective To investigate the influencing factors of successful extubation and withdrawal in patients with primary cerebral hemorrhage and respiratory failure. Methods From January 2017 to June 2018, 82 patients with primary cerebral hemorrhage complicated with respiratory failure were selected from the Changsha Fourth Hospital ("our hospital" for short). They were all treated with invasive mechanical ventilation in our hospital. They were divided into success group (n = 63) and failure group (n = 19) according to whether the extubation and withdrawl was sucessful. The clinical data of the two groups were compared. Logistic regression was used to analyze the influencing factors of successful extubation and withdrawal in patients with primary cerebral hemorrhage and respiratory failure. Results The successful rate of extubation and withdrawal was 76.83% and the failure rate was 23.17%. The age, bleeding volume, history of chronic obstructive pulmonary disease (COPD) and National Institute of Health Stroke scale (NIHSS) score of the failure group were significantly higher than those of the success group (P < 0.05), while Glasgow Coma Scale score (GCS score) and serum albumin at extubation were significantly lower than those of the success group (P < 0.05); age, bleeding volume, history of COPD, and serum albumin at the time of extubation were the influencing factors for successful extubation and withdrawal in patients with primary cerebral hemorrhage complicated with respiratory failure (OR = 2.197, 1.578, 2.275, 0.582, P < 0.05). Conclusion The influencing factors of successful extubation and withdrawal in patients with primary cerebral hemorrhage and respiratory failure include age, amount of bleeding, COPD history and serum albumin level during extubation. The relevant influencing factors should be considered in the course of treatment and timely intervention.

100 Effect of urokinase intravenous thrombolysis and superselective artery th-rombolysis on acute middle cerebral artery infarction

Objective To investigate the effect of urokinase intravenous thrombolysis and superselective arterial thrombolysis in the treatment of acute middle cerebral artery cerebral infarction. Methods From April 2015 to June 2017, 104 patients with acute middle cerebral artery cerebral infarction admitted to the Second Department of Neurology, the People's Liberation Army Hospital 211, were divided into urokinase intravenous thrombolysis group (n = 52) and superselective arterial thrombolysis group (n = 52) according to different treatment methods. The National Institutes of Health Stroke Scale (NIHSS) score, coagulation function, vascular recanalization rate, satisfaction, and complications were observed and compared between the two groups before and 7 days after thrombolysis. Results There was no significant difference in NIHSS score between the two groups before thrombolysis (P > 0.05). The NIHSS score of the two groups on the 7th day after thrombolysis was lower than that before thrombolysis. The NIHSS score of the superselective arterial thrombolysis group on the 7th day after thrombolysis was lower than that of the urokinase intravenous thrombolysis group (P < 0.05). There was no significant difference in fibrinogen (FIB), prothrombin time (PT) and thrombin time (TT) between the two groups before and 7 days after thrombolysis (P > 0.05). The vascular recanalization rate, total effective rate and patient satisfaction of the superselective arterial thrombolysis group were higher than those of the urokinase intravenous thrombolysis group, and the incidence of complications was lower than that of the urokinase intravenous thrombolysis group, the differences were statistically significant (P < 0.05). Conclusion Superselective arterial thrombolysis can significantly improve the neurological function of patients with acute middle cerebral artery cerebral infarction. The thrombolytic effect is better and the complications are less. It is worthy of clinical application.

104 Effect of Dexmedetomidine on inflammatory factors and renal function in elderly patients undergoing hip replacement surgery

Objective To observe the effects of Dexmedetomidine on inflammatory factors and renal function in elderly patients after hip replacement treatment, and to provide basis for selection of anesthetic sedative medication for elderly patients undergoing hip replacement surgery. Methods A total of 200 elderly patients accepted hip replacement in Hanzhong Central Hospital of Shaanxi Province from September 2015 to December 2017 were selected and divided into experimental group and control group according to the random number table method, with 100 cases in each group. The patients in the experimental group were given 1 μg/kg intravenous infusion of Dexmedetomidine, after 10 min, the patients were maintained by 0.2 μg/(kg·h) Dexmedetomidine +Propofol; the control group were given intravenous injection of 0.9% sodium chloride solution + Propofol. Before anesthesia (T0), skin incision (T1), surgical (T2), the end of the postoperative 6 h (T3), the heart rate (HR), mean arterial pressure (MAP), and the serum tumor necrosis factor alpha (TNF-α), interleukin 6 (IL-6), c-reactive protein (CRP), creatinine (Cr) and blood urea nitrogen (BUN) level changes of the two groups were compared. The incidence of adverse reactions during the recovery period and the Ramsay sedation score at T0 and T3 were recorded and observed. Results The levels of MAP and HR as well as the levels inflammatory factors TNF-α, CRP and IL-6 at T1, T2 and T3 in the experimental group were significantly lower than those in the control group (P < 0.05). At T1, T2 and T3 time points, the levels of Cr and BUN in the experimental group was lower than that in the control group, and the differences were statistically significant (P < 0.05). The incidence of adverse reactions in the experimental group was lower than that in the control group, and Ramsay score was higher than that in the control group, with statistically significant differences (P < 0.05). Conclusion The application of Dexmedetomidine in elderly hip replacement can inhibit the expression of inflammatory factors and protect the kidneys, which can be used as an auxiliary anesthetic for hip replacement in the elderly.

Objective To observe the effect of painless sedation and anesthesia on blood pressure in patients with hypertension by endobronchial ultrasound-guided transbronchial needle aspiration biopsy (EBUS-TBNA). Methods From January 2016 to December 2017, 70 cases of hypertension patients with mediastinal and hilar occupying lesions on chest enhanced CT admitted to the Department of Respiratory Medicine of Jiangning Hospital Affiliated to Nanjing Medical University were selected. EBUS-TBNA examination was performed in 40 patients under painless sedation and anesthesia (PR group, n = 40), and EBUS-TBNA examination was performed in 30 patients under local anesthesia (L group, n = 30). The changes of blood pressure, heart rate and blood oxygen saturation in the two groups were observed when the two anesthesia methods were used for examination, and the anesthetic effect, subjective feelings and adverse reactions of the patients in the examination were evaluated. Results Blood pressure of patients in the L group increased during and after examination compared with that before anesthesia, and the difference was statistically significant (P < 0.05). The blood pressure of patients in the PR group decreased during and after the examination compared with that before anesthesia, and the difference was statistically significant (P < 0.05). The anesthesia effect of PR group was significantly better than that of L group, the diagnosis rate was significantly higher than that of L group, and the satisfaction rate of sampling was significantly lower than that of L group, the difference was statistically significant (P < 0.05). Bradycardia was more common in PR group than in L group, and no other serious complications were found in both groups. Conclusion Compared with local anesthesia, painless sedation anesthesia can significantly reduce patients′ intraoperative or postoperative blood pressure, which is more suitable for ebus-tbna examination in patients with hypertension.

117 Effects comparison of moxibustion in different habitats in the treatment of moderate and severe primary knee osteoarthritis

Objective To compare the effects of moxibustion in different habitats in the treatment of moderate and severe primary knee osteoarthritis. Methods From August 2016 to November 2017, 85 patients with knee osteoarthritis admitted to Outpatient Department of Acupuncture and Moxibustion of Beijing First Hospital of Integrated Chinese and Western Medicine were selected. Through logging on the central random system of clinical research of China Academy of Chinese Medical Sciences, the random group number was achieved, then the patients were randomly divided into treatment group 1 (34 cases), treatment group 2 (34 cases) and control group (17 cases). Treatment group 1 was treated with Qichun moxa, treatment group 2 was treated with Nanyang moxa and the control group was treated with oriental wormwood moxibustion, the treatment lasted for 2 weeks continously, the follow-up was perfomed at one month after the end of treatment. After treatment for 2 weeks and at the time of follow-up, the knee pain and the efficacy of clinical symptoms among the three groups were observed and compared. Results Before treatment, there were no significant differences in visual analogue scale (VAS), the pain score, stiffness score and functional activity score of Western Ontario McMaster University osteoarthritis index (WOMAC) among the three groups (P > 0.05). After treatment for 2 weeks, the difference values (compared with those before treatment, the same below) of VAS pain scores and WOMAC functional activity scores among the three groups had statistically significant differences (P < 0.05), but there were no significant differences in the difference values of WOMAC pain score and stiffness scores among the three groups (P > 0.05). At the follow-up of one month after the end of treatment, the difference values of VAS scores, WOMAC pain scores, WOMAC functional activity scores among the three groups had statistically significant differences (P < 0.05 or P < 0.01), while there was no significant difference in the difference values of WOMAC stiffness scores among the three groups (P > 0.05). Conclusion The treatment of all three groups can alleviate knee pain, improve the function of movement, among which, treatment group 1 may alleviates pain best, followed by treamtnet group 2, control group.

The compound decoction is complex, the compatibility and function is complex, to fit complex disease syndrome. The compatibility methods of the compound decoction are as followed: principle-method-recipe-medicines is conformable and prescription is corresponding to the syndrome; disease-syndrome-manifestation is conformable in drug use; according to choice of pulse, syndrome and manifestation to increase or decrease the corresponding drug. The compound decoction dispensing methods are as followed: medicine according to the compound generous theory guidance; the medicine corresponding to the syndrome and repeating drug use; focused drug use; combining the decoctions to one and overlapping the decoctions. Compound drug simplification methods are as followed: finding the main medicine for treatment; selecting small decoctions to form new prescription; application of decoction structure; application of fixed structure of drugs; flexible use of multiple compatibility; in reference of pharmacological research results of traditional Chinese medicine. The doctor needs wealthy theoretical knowledge and clinical experience so that can achieve a rational use of the compound decoction.

Ulcerative colitis (UC) is a kind of intestinal inflammation disease, which is chronic and refractory. The etiology of UC has not been defined yet. Its clinical treatment is complex. The quality of patients′ life is seriously affected. The contemporary famous traditional Chinese medicine doctors have accumulated rich clinical experience in the treatment of ulcerative colitis. They think that spleen deficiency and stagnated dampness are the basic pathogenesis of UC. The pathogenesis also involves qi stagnation, blood stasis, phlegm, cold dampness, epidemic toxin, hidden toxin, blood heat, etc. The types of syndromes mainly include liver stagnation and spleen deficiency syndrome, spleen qi deficiency syndrome, spleen deficiency and stagnated dampness syndrome, qi stagnation and blood stasis syndrome, yang deficiency of spleen and kidney syndrome, dampness heat of large intestine syndrome, excessive toxic heat syndrome, etc. The combination of clinical stages and syndrome differentiation is paid much attention, and elimination and tonifying method cooperated, therapeutics of taking orally and external treatment have achieved remarkable clinical efficacy.

141 Professor PENG Kang′s clinical experience on treating headache with scorpio concentrate granules

To summarize the clinical experience of Professor PENG Kang on treating headache with scorpio concentrate granules. Headache is a common clinical disease, with various etiologies and complicated mechanisms. Professor PENG thinks that patients with complex headache should be treated with tonifying qi and blood, eliminating phlegm and free channels, extinguishing wind to relieve pain for the law. Prescriptions on the basis of Buyang Huanwu Decoction, Buzhong Yiqi Decoction, Chaihu Shugan Decoction and Taohong Siwu Decoction are used. Professor PENG pays attention to treat from liver, and adds with insect medicine scorpion to relieve the liver and extinguish the wind, expel the wind to dredge collaterals. Professor PENG flexibly uses scorpio concentrate granules in treating headaches which enlightens the later studies.

145 The diagnose value of conventional ultrasound combined with injected subcutaneously contrast-enhanced ultrasound in sentinel lymph nodes of breast cancer

Objective To investigate the diagnose value of conventional ultrasound combined with injected subcutaneously contrast-enhanced ultrasound (CEUS) in transferred sentinel lymph nodes (SLN) of breast cancer. Methods Taking the coarse needle biopsy results as a gold standard, the results of conventional ultrasound and CEUS of 101 SLNs in the ipsilateral axilla of 66 breast cancer patients from March to December 2017 in the Affiliated Hospital of North Sichuan Medical College were retrospectively analyzed and compared. Different perfusion methods of lymph nodes in CEUS were used for scoring. The two methods were analyzed by Logistic regression and receiver operating characteristic (ROC) curve. Z test was used to evaluate the curve, McNemar test was used to compare the results of the two methods. The cut-off value was determined according to the ROC curve of CEUS and SLNs were assessed. Results The area under the ROC curve of the two methods was compared, the area under the ROC curve of conventional ultrasound was 0.779 and contrast-enhanced ultrasound was 0.772, the difference was not statistically significant (P > 0.05). The conventional ultrasound′s sensitivity, specificity and coincidence rate were 51.11%, 89.47%, 72.55% respectively. The CEUS′s sensitivity, specificity, coincidence rate were 46.67%, 93.1%, 72.82% respectively, there was no significant difference among the indicators (P > 0.05). From CEUS′s ROC curve, the cut off value can help to evaluate the benign and malignant SLN. Conclusion Subcutaneously contrast-enhanced ultrasound has a high practical value in evaluating the benign and malignant SLN, and it is a powerful supplement to axillary lymph nodes detected by conventional ultrasound.

149 Analysis and study of ultrasonographic characteristics of thyroid nodules

Objective To assess the value of ultrasonographic characteristics and thyroid imaging reporting and data system (TI-RADS) for qualitative diagnosis of thyroid nodules. Methods A total of 347 patients with thyroid nodules underwent surgical treatment in the Affiliated Huai′an No.1 Pecople′s Hospital of Nanjing Medical University from January to December 2016 were enrolled. All of the enrolled patients were divided into the benign group (n = 127) and the malignant group (n = 220) according to the postoperative pathological diagnosis results. The ultrasound imaging findings in the two groups were observed and compared. The patients were divided into 1 to 5 grades by TI-RADS classification standard, and the grading diagnosis results were compared with pathological diagnosis results. Results Of the 347 patients, there were 1 case of TI-RADS level 1, 54 cases of TI-RADS level 2, 59 cases of TI-RADS level 3, 51 cases of TI-RADS level 4a, 88 cases of TI-RADS level 4b, 68 cases of TI-RADS level 4c, 26 cases of TI-RADS level 5. Ultrasound images in the malignant group showed characteristics of irregular shape, unclear boundary, microcalcification, and abundant blood flow signals, the propotions of these characteristics were higher than those in the benign group, the differences were statistically significant (P < 0.05). Among the 347 patients, using grade 4a as the criterion for classifying benign and malignant lesion, the specificity, sensitivity and accuracy of TI-RADS classification was 74.8%, 91.4% and 85.3%, respectively; if only grade 1-3 (representing benign lesion) and grade 4b-5 (representing malignant lesion) were considered, with exclusion of the dubious grade 4a, the specificity, sensitivity and accuracy of TI-RADS classification was 91.3%, 90.1% and 90.1%, respectively. Conclusion The sensitivity, specificity and accuracy of TI-RADS classification in diagnosis of thyroid cancer are high. The application of TI-RADS classification is helpful for clinicians to interpret the diagnostic evaluation of ultrasound and thereby to make a reasonable treatment plan. So it has great clinical practical value.

153 Effect of Sulodexide on diabetic nephropathy

Objective To investigate the clinical effect of Sulodexide in the treatment for diabetic nephropathy (DN) patients. Methods A total of 200 patients with DN from January 2011 to January 2018 in Cangzhou Central Hospital, Hebei Province were enrolled in this study. They were divided into combined group and control group by random number table method, with 100 patients in each group. The control group was given basal therapy as hypoglycemic and microcirculation treatment. The combined group was treated with Sulodexide on the basis of the treatment in the control group. The fasting blood-glucose (FPG), blood glucose 2 h after meal (2 h PG), 24 h urinary albumin excretion rate (24 h UAER), serum creatinine (Scr), serum malonaldehyde (MDA), superoxide dismutase (SOD), the number of urinary podocytes, blood and urine monocyte chemoattractant protein 1(MCP-1) were compared before and after treatment. Results After treatment, 24 h UAER, Scr, MDA and urinary podocytes in both groups were significantly lower than those before treatment (P < 0.05), the SOD of the two groups were significantly higher than those before treatment (P < 0.05). After treatment, 24 h UAER, Scr, MDA and urinary podocytes in the combined group were lower than those in the control group (P < 0.05), and SOD was higher than that of the control group (P < 0.05). After treatment, blood and urine MCP-1 of the two groups were significantly lower than those before treatment (P < 0.05), and the blood and urine MCP-1 in the combined group were lower than those in the control group (P < 0.05). Conclusion Sulodexide in the treatment of DN can significantly reduce oxidative stress, blood and urine MCP-1 levels, urinary podocyte levels, and improve renal function.

157 Effects of different dose of Clopidogrel combined with Aspirin on GMP-140, platelet aggregation rate and safety in patients with unstable angina pectoris

Objective To explore the clinical efficacy and safety of different dose of Clopidogrel combined with Aspirin in the treatment of unstable angina pectoris (UAP). Methods From May 2016 to May 2017, 180 patients with UAP treated in Dongfeng Community Health Service Center of Chaoyang District, Beijing, and they were divided into A, B, C group, with 60 cases in each group. On the basis of Aspirin treatment, group A, B, C were treated with Clopidogrel 25, 50 and 75 mg/d orderly. The three groups were treated for 90 d. Efficacy and adverse reactions of the patients were retrospectively analyzed. Results After treatment, the level of GMP-140 in group B and C was lower than that in group A (P < 0.05), the rate of platelet aggregation induced by ADP was also lower than that in group A (P < 0.05), but there was no significant difference between group B and C (P > 0.05). The total effective rate of treatment in group B and C were higher than that in group A (P < 0.05), and the frequency of UAP in group B and C was less than that in group A, the duration of UAP was shorter in group B and C (P < 0.05), while there was no significant difference between group B and C (P > 0.05). There was no severe adverse reactions in the three groups, and there was no significant difference in the incidence of mild adverse reactions among the three groups (P > 0.05). Adverse cardiovascular events rate in group B and C was less than that in group A (P < 0.05). Conclusion Compared with other doses, 50, 75 mg/d Clopidogrel combined with Aspirin regimen in the treatment of UAP can get accurate effect, and it shows good safety.

161 Expression and significance of D-bifunctional protein in endometrial carcinoma

Objective To detect the expression level of D-bifunctional protein (DBP) in endometrial cancer tissues and its correlation with the detection results of serum tumor markers carcinoembryonic antigen (CEA) and carbohydrate antigen 125 (CA125), and to explore the clinical significance of DBP in the diagnosis and treatment of endometrial cancer. Methods From January 2013 to December 2015, 30 cases of patients undergoing endometrial cancer resection in Tangshan Maternal and Child Health Care Hospital (hereinafter referred to as "our hospital") were selected as the experimental group, and 20 cases of normal persons who underwent physical examination during the same period in our hospital were selected as the control group. Chemiluminescence was used to detect the expression levels of serum tumor markers CEA and CA125, and immunohistochemistry was used to detect the expression levels of DBP in endometrial and paracancer tissues, and the integral optical density value (IOD) was measured. Pearson correlation analysis was used to analyze the correlation between DBP and tumor markers CEA and CA125. Results The expression of IOD value of DBP in endometrial carcinoma was higher than that in adjacent tissues, and the difference was statistically significant (P < 0.05). There was a positive linear correlation between DBP and tumor markers CEA (r = 0.837, P < 0.01) and CA125 (r = 0.787, P < 0.01). Conclusion The expression of DBP is increased in patients with endometrial cancer and is closely related to the occurrence and development of endometrial cancer. The combined detection of CEA, CA125 and DBP has important significance for the early diagnosis of endometrial cancer, and provides a new research target for the treatment and prognosis of endometrial cancer.

164 Effect of continuous nursing with the emphasis of life style intervention in children and adolescents with type 1 diabetes mellitus

Objective To explore the effect of intensive lifestyle intervention on type 1 diabetes in children and adolescents Methods A total of 143 childrens and adolescents with type 1 diabetes admitted to the Department of Paediatrics and Endocrinology of the First Affiliated Hospital of Hebei North University from March 2014 to October 2016 were selected by convenient sampling method. According to the admission number of the patients, they were randomly divided into control group (74 cases) and intervention group (69 cases). At discharge, patients in the control group received routine discharge guidance, and the intervention group was combined with a 6 month continuous nursing mode focusing on intensive lifestyle intervention. At discharge, 3 months after intervention and 6 months after intervention, the quality of life scale (DQOLY), diabetes management self-efficacy scale (C-DMSES) and Likert five-level scale were used to evaluate the quality of life, self-management efficacy and compliance of the patients. Meanwhile, the number of readmission in the two groups within 6 months was calculated. Results Three months after discharge and 6 months after discharge, DQOLY evaluation of the intervention group was significantly better than that of the control group (P < 0.05), C-DMSES score was significantly higher than that of the control group (P < 0.05), and C-DMSES score of the two groups was significantly higher than that of the control group (P < 0.05), and compliance of the intervention group was significantly better than that of the control group (P < 0.05). In addition, within 6 months after discharge, the number of patients in the control group who were readmission due to non-compliance with medicine, readmission due to irregular diet, and readmission due to diabetic complications were significantly higher than those in the intervention group (P < 0.05). Conclusion Continuous nursing with the focus of intensive lifestyle intervention can significantly improve the quality of life of patients, improve their satisfaction and compliance, enhance the self-confidence of patients' self-management behavior, and effectively reduce the short-term readmission rate after discharge.

169 The clinical effect of fast track surgery concept on gastrointestinal function recovery after colon cancer resection

Objective To explore the application effect of fast-track surgery (FTS) concept in gastrointestinal function recovery after colon cancer resection. Methods A total of 99 patients who underwent colon cancer resection from May 2016 to February 2018 in Shengjing Hospital of China Medical University were selected as study subjects. Patients were randomly divided into control group (n = 49) and research group (n = 50) according to the random number table method. Patients in the control group were treated with routine perioperative management, and patients in the study group were treated with FTS nursing intervention on this basis. Clinical indexes and gastrointestinal function were compared between the two groups. Serum levels of interleukin-6 (IL-6), C-reactive protein (CRP) and serum amyloid protein A (SAA) were compared between the two groups before surgery, 3 days after surgery and 7 days after surgery. The postoperative complications of the two groups were observed. Results Hospitalization time, first anal exhaust time, first defecation time and recovery time of normal diet in the study group were all shorter than those of the control group, the differences were statistically significant (P < 0.05). However, there was no significant difference in intraoperative blood loss and operation time between the two groups (P > 0.05). 3, 7 days after surgery, IL-6, CRP and SAA levels of the two groups were higher than those before surgery, and IL-6, CRP and SAA levels at 7 days after surgery were lower than those 3 days after surgery. IL-6, CRP and SAA levels at 7 days after surgery were all lower than 3 days after surgery (P < 0.05). IL-6, CRP and SAA levels of the study group were lower than those of the control group 3, 7 days after surgery (P < 0.05). The incidence of postoperative complications in the study group was 10.00% (5/50), which was significantly lower than 30.61% (15/49) in the control group, and the difference was statistically significant (P < 0.05). Conclusion After colon cancer resection, FTS nursing intervention can significantly improve clinical indicators, promote the recovery of gastrointestinal function, and effectively improve patients′ inflammatory response, reduce the incidence of complications, which has certain clinical application value.

173 Effect of behavioral change combined with traditional healthy education in patients with type 2 diabetes mellitus complicated with cerebral infarction

Objective To investigate the effect of behavioral change combined with traditional healthy education in patients with type 2 diabetes mellitus complicated with cerebral infarction. Methods Clinical data of 100 patients with type 2 diabetes mellitus complicated with cerebral infarction admitted to West China Hospital, West China School of Medicine Sichuan University from March 2017 to March 2018 were retrospectively analyzed. According to different nursing modes, they were divided into two groups, 50 patients in the control group (traditional health education mode) and 50 patients in the observation group (behavior change measures). Blood glucose control knowledge score, self-efficacy behavior ability score, fasting blood glucose, 2 h postprandial blood glucose, glycosylated hemoglobin level, national institutes of health stroke scale (NIHSS) score and daily living ability scale (ADL) score before and after intervention were observed in the two groups, and patient satisfaction in the two groups was observed. Results Patients in the two groups were compared in pre-intervention glycemic control knowledge score, self-efficacy behavioral ability score, fasting blood glucose, postprandial 2 h blood glucose and hemoglobin level, NIHSS score and ADL score, there were no significant differences between the two groups (P > 0.05). After intervention, the glycemic control knowledge score, self-efficacy behavior ability score and ADL score of patients in the two groups were all higher than those in the same group before intervention, and the fasting blood glucose, 2 h postprandial blood glucose, hemoglobin level and NIHSS score were all lower than those in the same group before intervention. In addition, patients in the observation group were higher than those in the control group in terms of glycemic control knowledge score, self-efficacy behavior ability score and ADL score after intervention, fasting blood glucose, 2 h postprandial blood glucose, hemoglobin level and NIHSS score, and the satisfaction of patients in the observation group was higher than that in the control group, the differences were statistically significant (all P < 0.05). Conclusion The application of behavioral change combined with traditional healthy educaticon in patients with type 2 diabetes mellitus complicated with cerebral infarction can improve patients′ knowledge of blood glucose control, self-efficacy, improve blood glucose level, reduce nerve function injury, and improve life ability and satisfaction, which is worthy of clinical promotion and application.

177 Nursing care of 15 patients with acute limb ischemia after cardiac surgery

Objective To summarize the nursing experience of 15 patients with acute limb ischemia after cardiac surgery. Methods The nursing materials of 15 patients with acute limb ischemia from May 2011 to February 2018 in the Department of Cardiac Surgery of Peking Union Medical College Hospital were reviewed, by observing subjective and objective indicators, the experience and effect of nursing measures in limb perfusion, anticoagulant therapy, rehabilitation and discharge guidance were summarized, and the nursing emphases different from other departments were put forward. Results There were 15 cases of early postoperative ecchymosis, bruising, and cyanosis in 10 cases without progressive exacerbation, postoperative progressive exacerbation. There were 5 cases of ulcer/gangrene, including 3 cases of upper limb involvement, 2 cases of left/right foot and lower limb involvement. After careful management by the medical staffs, 10 of the 15 patients recovered well by the time they were discharged, by the time of discharge, there were still 5 cases of purpura and ulcer/gangrene, 2 of which required discharge. Conclusion By summarizing the nursing experience of 15 patients with acute limb ischemia, it can provide reference for clinical nursing and promote the recovery of patients.
