Effects of curcumin on apoptosis after human cytomegalovirus infection in human embryonic pulmonary fibroblasts

Objective To investigate the apoptosis of human embryonic lung fibroblast cells infected with human cytomegalovirus (HCMV) and the effects of curcumin on apoptosis after HCMV infection. Methods The cells were divided into cell control group (cell maintenance solution), virus model group (cell maintenance solution + HCMV), high dose (cell maintenance solution + HCMV +0.8 μg/mL of curcumin ), middle dose (cell maintenance solution + HCMV + 0.4 μg/mL of curcumin) and low dose (cell maintenance solution + HCMV + 0.2 μg/mL of curcumin) of curcumin group, Ganciclovir group (cell maintenance solution + HCMV + 50 μg/mL of Ganciclovir). The effects of curcumin on apoptosis of HCMV infection were detected by mitochondrial membrane potential detection kit (JC-1) and fluorescent dye propidium iodide (PI) method. Results The JC-1 experimental results show that in the early stage of HCMV infection, there was no significant difference on cell apoptosis between virus model group and cell control group (P > 0.05); compared with virus model group, the apoptosis of Ganciclovir group and high dose, middle dose and low dose of curcumin group increased significantly (P < 0.05). In the late stage of HCMV infection, compared with cell control group, the apoptosis of virus model group increased significantly (P < 0.05); compared with virus model group, the cell apoptosis of high dose, middle dose and low dose of curcumin group decreased in different degrees, among them, the difference between the high dose of curcumin group and the virus model group was statistically significant (P < 0.05). The fluorescent dye PI results showed that in the early stage and late stage of HCMV infection, compared with cell control group, the number of host cells in the S phase in virus model group increased significantly (P < 0.01); compared with virus model group, the number of host cells in S phase in high dose, middle dose and low dose of curcumin group decreased significantly (P < 0.05). Conclusion Curcumin has antiapoptotic activity in HCMV infection and can weaken anti-apoptosis induced by early HCMV infection and pro-apoptosis induced by late HCMV infection.

Effect and the possible mechanisms of Shensong Yangxin on myocardial microcirculation in rabbits with myocardial infarction

Objective To explore the effect and possible mechanism of Shensong Yangxin on myocardial microcirculation in rabbits with myocardial infarction. Methods Thirty 6-month-old SPF grade Japanese large ear rabbits were randomly divided into the Sham group, Control group and MI+SSYX group via random number table method, with 10 rabbits in each group. Each group consists of equal number of male and female rabbits. Rabbit models with myocardial infarction in Control group and MI+SSYX group were established by ligation of left anterior descending coronary artery. Rabbits in Sham group were applied stitches under the coronary artery without ligation. After the surgery, rabbits in MI+SSYX group were administered 0.4 g/(kg·d) of Shensong Yangxin dissolved in saline. The Sham group and Control group were treated with the same dose of saline. After 4 weeks of administration, the rabbits of each group were sacrificed and CD34 in the peripheral area of myocardial infarction was tested by immunofluorescence staining to measure the microvessel density. The plasmic levels of endothelin-1 (ET-1), thromboxane A2 (TXA2), nitric oxide (NO) and von Willebrand factor (vWF) were examined by enzyme-linked immunosorbent assay (ELISA). Results Compared with Sham group, the microvessel density were significantly decreased in Control group and MI+SSYX group(P < 0.05); compared with the Control group, the microvessel density was significantly increased in MI+SSYX group(P < 0.05). Compared with Sham group, the plasmic levels of ET-1, NO, TXA2 and vWF in Control group and MI+SSYX group were significantly increased (P < 0.05); compared with Control group, the plasmic levels of ET-1, TXA2, vWF were decreased and the level of NO was increased in MI+SSYX group (P < 0.05). Conclusion Shensong Yangxin can improve myocardial microcirculation in rabbits with myocardial infarction. The mechanism may be related to the functions that Shensong Yangxin can promote the generation of capillary in the boundary of myocardial infarction and balance the endothelial-derived vasodilators and vasoconstrictors.

Effects of curcumin on the expression of PTP1B and IRS-2 mRNA in liver tissue of rats with type 2 diabetes mellitus

Objective To observe the effects of curcumin on expression of protein tyrosine phosphatase 1B (PTP1B) and insulin receptor substrate-2 (IRS-2) mRNA in liver tissue of rats with type 2 diabetes mellitus induced by high fat diet. Methods The SD rats were fed with high fat diet, then the tail vein was injected with Streptozotocin (STZ), so as to make type 2 diabetes mellitus model. The model rats were divided into model group, crucumin group, Pioglitazone group, with 10 rats in each group. After 4 weeks and 8 weeks of administration, the levels of fasting blood-glucose (FBG), fasting insulin (FINS), serum total cholesterol (TC), triacylglycerol (TG), low density lipoprotein cholesterin (LDL-C), high density lipoprotein cholesterol (HDL-C) were detected, and the liver index (LI), insulin sensitivity index (ISI) were calculated. After 8 weeks of administration, the pathological changes were observed by HE staining of liver, the mRNA expression of PTP1B and IRS-2 in liver was detected by real-time PCR. Results Compared with model group, crucumin group could significantly reduce the weight and the serum levels of FBG, FINS, LDL-C of rats with type 2 diabetes mellitus (P < 0.05), increase the insulin sensitivity, improve the fatty degeneration and inflammatory reactions of liver tissue, inhibit the expression of PTP1B mRNA, and up-regulate the expression of IRS-2 mRNA (P < 0.05). Conclusion Curcumin can improve the insulin resistance of model rats with type 2 diabetes mellitus, the mechanism may be related to inhibition of PTP1B mRNA expression and up-regulation of IRS-2 mRNA expression.

Study on the reversal effect and mechanism of tetrandrine for Cisplatin-resistant breast cancer cells based on ATP7A

Objective To explore the reversal effect and mechanism of tetrandrine for Cisplatin-resistant breast cancer cells based on ATP7A. Methods Two kinds of human Cisplatin-resistant breast cancer cells lines and cells with high expression of ATP7A were established to detect the reversal effects of tetrandrine on Cisplatin-resistant breast cancer cells. The intracellular content of Cisplatin was determined by high performance liquid chromatography and the expression of ATP7A protein was detected. Results The sensitivity of Cisplatin in resistant breast cancer cells was significantly decreased (P < 0.05), the expression of ATP7A protein was increased (P < 0.05), and the intracellular content of Cisplatin was decreased significantly (P < 0.05); compared with normal breast cancer cells, the sensitivity of Cisplatin in cells with high expression of ATP7A significantly decreased (P < 0.05), and the intracellular content of Cisplatin was decreased significantly (P < 0.05). Tetrandrine had significant reversal effects for the drug resistance of cells, which could increase the intracellular content of Cisplatin (P < 0.05), and decrease the expression of ATP7A compared with drug-resistant cells and transfected cells (P < 0.05). Conclusion Tetrandrine has reversal effects for human Cisplatin-resistant breast cancer cells lines and cells with high expression of ATP7A, its mechanism may be related to the inhibition of ATP7A expression.

Establishment of microbial limit test for hospital ear and nose preparations

Objective To establish the method of microbial limit test for 3 kinds of hospital ear and nose preparations. Methods According to the provisions of the 2015 edition of the Pharmacopoeia of the People′s Republic of China (Part Ⅳ) microbiological limit inspection method, the applicability test was carried out on the microbial limit test method of the three kinds of hospital nasal drop and ear drops by using the plate method. Results The method established in this study has a recovery rate of 0.5-1.5 for five kinds of proven bacterial strains of Staphylococcus aureus, Pseudomonas aeruginosa, Bacillus subtilis, Candida albicans and Aspergillus niger. Control bacteria (Escherichia coli, Pseudomonas aeruginosa, Staphylococcus aureus) could be detected, but control bacteria were not detected in negative control group. Conclusion The microbial limit test of boric acid ear drops, compound furacilin nasal drops and compound menthol nasal drops was carried out by plate method. The operation was simple, rapid, the results were accurate and reliable. It could objectively reflect the contamination of microorganisms in drugs and better control the quality of three kinds of hospital ear and nose preparations.

Research on total polyphenol content of different ethanol fractions from Xanthoceras sorbifolia husks and the antioxidant activity

Objective To establish the determination method of total polyphenol content (TPC) of the ethanol extract from X. sorbifolia husks, and evaluate the TPC and antioxidant activity in vitro of different ethanol fractions from X. sorbifolia husks. Methods The TPC of X. sorbifolia husks was analyzed by spectrophotometry. The antioxidant activity of different ethanol fractions from X. sorbifolia husks was evaluated by ABTS and DPPH methods. Results The TPC of X. sorbifolia husks was 10.22 mg/g. After purified by D101 macroporous resin, the polyphenols from X. sorbifolia husks were mainly concentrated in the 30% ethanol fraction, the TPC of which was 4.92 mg/g. This faction exhibited strong antioxidant activity, the IC50 values against ABTS and DPPH free radicals of which were (4.83±0.33), (9.33±0.23) μg/mL respectively. While the IC50 values of the positive control rutin against ABTS and DPPH free radicals were (12.87±0.21), (10.29±0.17) μg/mL respectively. Conclusion The 30% ethanol fraction purified by D101 macroporous resin has the highest TPC and the strongest antioxidant activity, which is worthy of further development and utilization.

Identification of raw and processed atractylodes by DCBI-MS

Objective To establish a rapid identification method for desorption corona beam ionization-mass spectrometry (DCBI-MS) (second order) of processed and unprocessed atractylodes. Methods The in situ tracking technique was used to track the transformation of atractyl ketone, and the best processing temperature of atractylodes were discussed. The chemical constituents of processed and unprocessed atractylodes were analyzed by DCBI-MS. Results The best processing temperature of atractylodes were 120-140℃. Sixty batches of samples included 1 batch of reference medicinal materials, 27 batches of raw medicinal herbs, 19 batches of earth-boiled atractylodes, and 13 batches of bran-enriched atractylodes; compared with raw products, the contents of atractyl ketone in processed products decreased to varying degrees, and the relative abundance of atractylenolide Ⅱ and atractylenolide Ⅲ increased significantly; the similarity of raw atractylodes was 0.90-0.95, the similarity of atractylodes processed with soil was 0.83-0.90, and the similarity of atractylodes processed with wheat bran was 0.71-0.82, and it can be used to visually distinguish between raw atractylodes, atractylodes processed with soil, and atractylodes processed with wheat bran. Conclusion The DCBI-MS method can effectively track the transformation of atractylenolide Ⅰ, Ⅱ and Ⅲ during processing, and obtain the best processing parameters, and can effectively distinguish the processed and unprocessed atractylodes, which can be optimized as processing conditions. The rapid detection tool provides a reference for the quality control of Chinese herbal medicines.

The function and application prospect of miRNAs in hepatocellular carcinoma

Hepatocellular carcinoma (HCC) is the most common primary malignant tumor of the liver and is one of the leading causes of cancer deaths in the world, it is characterized by a poor prognosis. More and more researches have focused on finding new molecular markers for HCC diagnosis and prognosis. microRNAs (miRNAs) is a sort of small and non-coding RNA. Deregulated miRNAs themselves act as oncogenes or tumor suppressor genes and are involved in the development and progression of cancer. Recent studies have found that the abnormal expression of many miRNAs is involved in many biological processes of HCC. This paper summarizes the studies on the role of miRNAs in the development and progression of HCC tumors in recent years, and provides a theoretical basis for the application of miRNAs in HCC diagnosis, prognosis or as a therapeutic target.

Research progress on the diagnosis criteria and treatment strategy of adolescent polycystic ovary syndrome

Polycystic ovary syndrome (PCOS) is the most common endocrine disorder in women of reproductive age, and the onset of PCOS is in puberty. Adolescent PCOS is a disease concerned by gynecologists and pediatric endocrinologists because of overlapping clinical features and physiological manifestations of puberty, the diagnostic criterion for adolescent PCOS is still controversial. At present, guidelines for the diagnosis and treatment of polycystic ovarian syndrome in China suggests that diagnostic criterion for adolescent PCOS should fulfill all the three indexes of 2003 Rotterdam PCOS diagnostic criterion and exclusion of other causes of elevated androgen levels and other diseases that cause ovulation disorders. Treatment strategy of adolescent PCOS should be focused on using multiple treatments to adjust menstrual cycle, correct abnormal sex hormones and glucolipid metabolism, improve other clinical symptoms and prevent long-term complications, according to different clinical symptoms and signs. In addition to lifestyle intervention, oral contraceptives, insulin sensitizer and so on, complementary and alternative medicine managements such as natural therapies, yoga asana practice, acupoint catgut embedding and auricular acupressure are also used in treating adolescent PCOS and have obtained good curative effects. The present study reviewed the progress of diagnosis and treatment on adolescent PCOS at home and abroad.

Advances in the study of function and clinical application of exosomes

Exosomes are membranous vesicles secreted by cells, which are important mediators for cell communication and participate in the transmission of biological signals between cells. There are five recognized and more commonly used methods of extracting exosomes. The breakthrough of exosomes in clinical research mainly reflects on that exosomes are involved in the development of tumor diseases, nucleic acids or proteins in exosomes can be used as molecular markers of diseases, and exosomes can be used as drug carriers for targeted therapy. The function of exosomes in diseases gives them an advantage in clinical application.This review focuses on the discovery of exosomes, the advantages and disadvantages of exosomes extraction methods and the research progress of exosomes function in
clinical diseases.

Advances in vitro studies on induction of autophagy in hepatocellular carcinoma by active ingredients of traditional Chinese medicine

Autophagy is a highly conservative self-renewal mechanism of cells, which is a core part of many physiological and pathological processes of the body. Autophagy has a double-edged effect on tumor regulation. It inhibits early tumors and promotes advanced tumors. Hepatocellular carcinoma (HCC) has high recurrence rate and poor prognosis, and there is no effective treatment for it currently. Throughout the literatures, many active ingredients of anti-cancer Chinese herbal medicine can increase the autophagy level in HCC cells. Some of them induce autophagic death to inhibit tumor, some of them trigger protective autophagy to support tumor survival and weaken the anti-cancer effects of drugs. In this paper, the research progresses of Chinese herbal medicine active components inducing HCC autophagy are reviewed, which provides new insights for the development of anti-HCC drugs targeting autophagy.

Research progress on the regulation mechanism of miRNA let-7 on diseases

MicroRNA (miRNA) is a highly conserved single-stranded small molecule RNA, which can regulate the growth, development, differentiation and apoptosis of histocytes. The let-7 miRNA family is currently the most widely studied family of miRNAs. Let-7 can be stably present in human peripheral blood and body fluids, and its expression and maintenance of equilibrium state are achieved by the relevant effects of regulatory factors. Abnormalities in these regulatory pathways lead to abnormal expression of the let-7 family, which leads to the development of tumors, hypertension, epilepsy, and cardiovascular diseases. Therefore, monitoring the expression level of let-7 can prevent and treat diseases associated with it. This review focuses on the research progress of miRNA let-7 on the regulation of these diseases.

Research progress on the teaching status of fitness Qigong Ba Duan Jin

This study analyzed the literature of Ba Duan Jin in the past 15 years, and comprehensively elaborated from its training population, teaching constraints, the status quo of teachers, the theoretical support of teaching, teaching methods and the application of different teaching modes in Ba Duan Jin teaching. The whole responsibility system teaching mode proposed by our group applied the "four step" training method, which is more suitable for the promotion of Ba Duan Jin in primary hospitals. Ba Duan Jin can not only be used as a fitness method for ordinary people, but also a prescription for exercise for most patients. In the teaching process, multi-pronged measures, teaching in accordance with their aptitude, using modern video and assessment tools to create the most suitable exercise amount and exercise intensity for the practitioners, to stimulate the participants to participate in the awareness, and truly improve the teaching effect and provide reference for the promotion of Ba Duan Jin in the primary hospitals.

Application of animal models of renal interstitial fibrosis in experimental teaching of gene regulation

Objective To explore the application of animal models of renal interstitial fibrosis in experimental teaching of gene regulation. Methods From September 2016 to September 2017, 60 undergraduate students from 2012 and 2013 medical biotechnology majors in Army Military Medical University were selected as the research subjects. The students were randomly divided into two groups, 30 students in each group. The conventional method was used in the control group for gene regulation experiment teaching, while the renal interstitial fibrosis animal model to carry out gene control experiment teaching was used in observation group. Then rated the teaching situation of teachers and students in two groups, students′ final exam results and students′ evaluation of teaching effect. Results The scores of teaching process, teaching guidance and teaching effect of teachers and students in the observation group were higher than those in the control group, the differences were statistically significant (P < 0.05 or P < 0.01). The scores of theoretical examination and practical skill examination in the observation group were higher than those in the control group, the differences were highly statistically significant (P < 0.01). The students′ overall impression of teaching methods, cultivation of experimental practical ability, students′ good grasp of theoretical knowledge through experimental courses, experimental ability to cultivate students′ innovative thinking, cultivate students′ team cooperation ability, and the satisfaction rate of accepted teaching methods in the observation group were higher than those in the control group, the differences were highly statistically significant (P < 0.01). Conclusion The application of animal models of renal interstitial fibrosis in experimental teaching of gene regulation can improve students′ interest in learning and academic performance, and improve teaching quality.

Influence of the master system on the development of the traditional health-care P.E. course of universities of Chinese medicine

The master system, as a kind of cultural phenomenon, is widely existed in the field of cultural skills. The master-prentice is the institutional guarantee for the inheritance of traditional health-care sports culture. The system suggests that the master system has defined the teachers′ responsibility of traditional health sports, ensured the quality of traditional health sports inheritors, and strengthened the cohesion of the inheritors. This system also advises that the master system restricts the widespread dissemination of traditional health-care techniques, and breeds the narrow prejudice of traditional health-care sports units. This paper provides the reference for re-examining the positioning and influence of master system in the traditional health-care sports.

Practice and thinking of military-civilian integration and cross-discipline training methods for military medical postgraduates

Military graduate students not only need to take the responsibility of protecting the country, but also need to devote themselves to healing the wounded and doing scientific research. With the development of science and technology, modern medicine has been increasingly showing the tendency of integration between military-civilian integration and interdisciplinary. However, the cultivation of medical postgraduates under past military school management model is far from enough to meet the demands of this new age. The methods of improving the quality of medical postgraduate education in military academies are urgently needed. This article analyzes the existing problems in the cultivation of graduate students in military medical schools from the aspects of the teaching staff, student composition, management work and evaluation mechanisms. It also elucidates the advantages of military-civilian integration in promoting cross-discipline training. Combining with the exploratory research of the Southwest Hospital of Army Medical Uniuersity, we put forward the thinking of military-civilian integration and cross-discipline training methods for postgraduate medical graduates of this new age, which aim to promote the training quality of military medical postgraduates and the development of China′s national defense medical and health undertakings.

Application value of Mini-CEX examination model in clinical teaching of brain surgery interns

Objective To study the application value of mini-clinical exercise assessment (Mini-CEX) model in the clinical teaching of brain surgery interns. Methods From January to June 2018, 100 intern doctors performed in the Department of Neurosurgery, Dongfang Hospital of Beijing University of Chinese Medicine were selected, according to the random number table method, they were divided into control group and observatiou group, with 50 cases in each group. In the control group, the intern received the traditional teaching mode, and the intern in observation group received the Mini-CEX examination model. The results of clinical comprehensive ability assessment, clinical teaching approval, teaching satisfaction and stress index in the two groups were compared. Workplace mindfulness score, career satisfaction score in two groups were compared. Results The scores of medical interview expression skills, physical examination skills, clinical comprehensive judgment, doctor-patient communication skills, organizational efficacy, clinical professional attitude and overall clinical ability in the observation group were higher than those in the control group, the differences were statistically significant (P < 0.05). In the aspects of improving learning enthusiasm, learning efficiency, autonomous learning ability, problem finding ability and problem-solving ability, the recognition of clinical teaching in the observation group was higher than that in the control group, the differences were statistically significant (P < 0.05). The total satisfaction rate of clinical teaching in the observation group was higher than that in the control group, the difference was statistically significant (P < 0.05). The stress index of the two groups after teaching was lower than that before teaching, the scores of mindfulness and satisfaction in the workplace after teaching in both groups were higher than those before teaching, the differences were statistically significant (P < 0.05); After teaching the stress index of the intern in the observation group was lower than that in the control group, and the scores of mindfulness and satisfaction were higher than those in the control group, the differences were statistically significant (P < 0.05). Conclusion In the clinical teaching of brain surgery interns, the application of Mini-CEX examination model in teaching can effectively improve the clinical comprehensive ability of brain surgery interns, and help to improve their recognition and satisfaction with clinical teaching. Application of PBL combined with CBS method in clinical laboratory hematology technology teaching

In order to improve and enhance the teaching effect, we should pay attention to the improvement of teachers′ teaching level, give full play to the role of the network platform, combine with traditional teaching, and build small-sized class. PBL combined with CBS teaching mode can not only improve students′ learning effect, but also promote teachers′ teaching ability and level, which is worthy of popularization and application.

Application of suspended core stabilization exercises in the treatment of chronic nonspecific low back pain

Objective To investigate the clinical suspended core stabilization exercises in the treatment of patients with chronic nonspecific low back pain. Methods Ninety-six patients with chronic nonspecific low back pain treated in Beijing Rehabilitation Hospital, Capital Medical University from January 2016 to January 2017 were randomly divided into observation group and control group by random number table method, with 48 cases in every group. The observation group was given core stabilization exercises by a sling on the basis of extracorporeal shock wave therapy, and the control group was treated with extracorporeal shock wave only. The two groups were treated for 2 months. Before treatment, 2 months of treatment, and 6 months after treatment, visual analogue scale (VAS) was used to assess the degree of pain. Oswestry disability index (ODI) was used to evaluate lumbar function. The 36-item short form (SF-36) was used to evaluate the overall health status of patients, and the effective rate was calculated at 2 months of treatment. The occurrence of adverse reactions was recorded during the course of treatment. The recurrence of the two groups was recorded during the follow-up period 1 year after the end of treatment. Results There were no significant differences in VAS, ODI and SF-36 scores between the two groups before treatment (P > 0.05). The VAS score and ODI of the two groups at 2 months for treatment and 6 months after treatment were lower than before treatment, and the SF-36 scores were higher than before treatment (P < 0.05 or P < 0.01), and the observation group had significant changes compared with the control group (P < 0.05 or P < 0.01). The treatment effective rate at 2 months for treatment in observation group (83.33%) was significantly higher than that of the control group (60.42%) (P < 0.05). One year after treatment, the recurrence rate in the observation group (2.08%) was significantly lower than that of the control group (18.75%) (P < 0.05). No serious adverse reactions occurred in both groups during the treatment. Conclusion The use of core stabilization exercises by a sling in patients with chronic nonspecific low back pain can significantly reduce lumbar pain and enhance lumbar function, and it is safe and stable.

Predictive value of neutrophil gelatinase-associated lipocalin for continuous renal replacement therapy in septic patients with acute kidney injury

Objective To explore the clinical predictive value of neutrophil gelatinase-associated lipocalin (NGAL) in selecting continuous renal replacement therapy (CRRT) for septic patients combined with acute kidney injury (AKI). Methods Sixty-eight cases of sepsis patients combined with AKI admitted to Intensive Care Unit of Emergency of the Seventh People′s Hospital of Shanghai University of Traditional Chinese Medicine from January 2014 to December 2017 were selected. Serum NGAL (sNGAL) and urine NGAL (uNGAL) samples were collected within 24 h after diagnosis of AKI, and detected by ehzyme-linked immuno sorbent assay (ELISA) method. Serum creatinine (Scr) level and APACHE Ⅱ score were detected at the same time. According to whether patients needed CRRT treatment, they were divided into sepsis CRRT group (32 cases) and sepsis non CRRT group (36 cases). The differences of sNGAL, uNGAL, Scr and APACHE Ⅱ scores between the two groups were compared. By drawing the ROC curve, the predictive value of sNGAL, uNGAL and their combination in the early stage of sepsis combined with AKI in patients with CRRT was evaluated. Results There were no significant differences in the age, sex, pathogenesis and Scr level between the two groups on the day of sepsis combined with AKI (P > 0.05). However, the scores of sNGAL, uNGAL and APACHE Ⅱ in sepsis CRRT group were higher than those in non CRRT group (P < 0.05). ROC curve analysis showed that sNGAL, uNGAL and APACHE Ⅱ score in predicting sepsis combined with AKI patients needed CRRT treatment (95%CI), the area under the curve (AUC) was 0.863, 0.899 and 0.679, respectively. The prediction value of sNGAL combined with uNGAL was the largest and AUC was 0.932. Conclusion In the early stage of sepsis combined with AKI, sNGAL and uNGAL can be used as predictors of whether patients need CRRT treatment. The predictive value of combination of the two is more valuable.

Effect of Ziprasidone combined with Olanzapine therapy on myocardial zymogram and brain neurotransmitters in patients with first-episode schizophrenia

Objective To investigate the effect of Ziprasidone combined with Olanzapine therapy on myocardial zymogram and brain neurotransmitters in patients with first-episode schizophrenia. Methods One hundred and sixteen patients with first-episode schizophrenia admitted to the Department of Psychiatry, Lishui Second People′s Hospital from July 2016 to October 2017 were enrolled. The prospective study method was used and the patients were divided into treatment group and control group according to the random number table method with 58 cases in each group. The control group was given Olanzapine, and the treatment group was given Ziprasidone combine with Olanzapine orally. After 8 weeks of treatment, clinical efficacy, myocardial zymogram, neurotransmitter, and adverse reaction rate were compared between the two groups. Results The effective rate of the treatment group was 94.83%, which was significantly higher than 82.76% of the control group (P < 0.05). The serum creatine kinase isoenzyme-MB (CK-MB), lactate dehydrogenase (LDH) and sspartate aminotransferase (AST) of the treatment group were significantly lower than the control group (P < 0.05), while dopamine (DA) and γ-aminobutyric acid (GABA) were significantly higher than the control group, acetylcholine (Ach) and deep inhibitory neurotransmitters (INH) were significantly lower than the control group (P < 0.05 or P < 0.01). There was no significant difference in adverse reaction rate between the two groups (P > 0.05). Conclusion The combination therapy of Ziprasidone and Olanzapine can improve the clinical efficacy of patients with first-episode schizophrenia, and it is safe and reliable. The mechanism may be related to the factors such improving myocardial zymogram and regulating brain neurotransmitters.

Effect of laser irradiation combined with halogenat asone cream on vitiligo

Objective To investigate the effect of laser irradiation combined with halomethasone cream on vitiligo and its effects on interleukin-10 (IL-10) and interleukin-17 (IL-17) in the lesion area. Methods The clinical data of 98 patients with vitiligo admitted to the Department of Dermatology, Huangshi Central Hospital from June 2015 to September 2017 were retrospectively analyzed. According to the different treatment methods, they were divided into the observation group and the control group, with 49 cases in each group. The control group was treated with laser irradiation, and the observation group was combined with the halomethasone cream for external use on the basis of the control group. The therapeutic effects, adverse reactions and the concentrations of IL-10 and IL-17 in the lesions before and after treatment were compared between the two groups. Results The total effective rate of the observation group was higher than that of the control group (P < 0.05). After treatment, the IL-10 concentration in the lesion area was higher than that before treatment, and the IL-17 concentration was lower than that before treatment (P < 0.05). The IL-10 concentration in the lesion area of the observation group was higher than that of the control group, and the IL-17 concentration was lower than that of the control group (P < 0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P < 0.05). Conclusion The effect of laser irradiation combined with halomethasone cream on vitiligo is significant, which can effectively reduce IL-17 concentration and increase IL-10 concentration in the lesion area, and it is safe and reliable.

Association of iron overload with disease related indexes in non-diabetic patients with non-alcoholic fatty liver disease

Objective To investigate the association between iron overload and non-alcoholic fatty liver disease (NAFLD) in non-diabetic patients. Methods From January 2016 to December 2017, 152 patients with NAFLD who were not Complicated with diabetes enrolled in the Third Affiliated Hospital of Guangxi Medical University (“our hospital” for short) were selected as the observation group. Eighty healthy subjects udertaking phylical examination in our hospital were selected as the control group. Their general data, biochemical indicators, serum ferritin (SF), serum iron (SI) and serum hepcidin (HEPC) were recorded or detected, liver fat content (LFC) was evaluated by proton magnetic resonance spectroscopy. The relationships between the indexes of iron overload and other indexes were analyzed. Results SF and SI in the observation group were significantly higher than those in the control group, and HEPC was significantly lower than that in the control group, the differences were statistically significant (P < 0.05). After adjustment for age and sex, both SF and SI had linear positive correlations with LFC (r = 0.461, P < 0.001; r = 0.275, P = 0.001), and HEPC had a significant linear negative correlation with LFC (r = -0.479, P < 0.001). The increase of SI was an independent risk factor for liver damage (OR = 1.074, P < 0.05, 95%CI = 1.022-1.130). SF, SI, HEPC were weakly correlated with NAFLD activity scores (r = 0.172, P = 0.036; r = 0.192, P = 0.019; r = -0.236, P = 0.004), but the correlation coefficients were too low, which may lacked clinical significance. Conclusion Iron overload in non-diabetic patients is related to the increase of LFC and liver function damage, but it has no correlation with the degree of liver inflammation.

Contrastive study of the effect of Rocuronium Bromide and Cisatracurium Besilate on hemodynamics and stress response in patients with radical mastectomy of breast cancer

Objective To compare the effect of Rocuronium Bromide and Cisatracurium Besilate on anesthesia induction in patients with radical mastectomy of breast cancer. Methods A total of 97 patients who received radical mastectomy for breast cancer in NO.215 Hospital of Shaanxi Nuclear Industry from March 2015 to March 2018 were selected as study objects. According to the methods of anesthesia induction, patients were divided into research group (45 cases) and control group (52 cases). The anesthesia induction drug of the research group was Rocuronium Bromide, and the anesthesia induction drug of the control group was Cisatracurium Besilate. The anesthesia effect time, anesthesia time, tracheal extubation time were compared between the two groups; levels of hemodynamics, stress related index before anesthesia induction (T1), after anesthesia induction (T2), and the time of 3 min after laryngeal mask placement (T3) were compared between the two groups respectively; the adverse reactions of two groups were observed. Results The effective time of anesthesia in the research group was significantly lower than the control group, while the duration of anesthesia and the duration of trachea extubation were significantly higher than the control group (P < 0.05). At T2 and T3, the mean arterial and heart rate of the research group were significantly lower than the control group (P < 0.05). The levels of epinephrine and norepinephrine in the research group at T2 and T3 were significantly lower than the control group (P < 0.05 or P < 0.01). The incidence of adverse reactions in the research group was 8.89%, which was significantly lower than 28.85% of the control group (P < 0.05). Conclusion Compared with Cisatracurium Besilate, Rocuronium Bromide is more effective in induction of anesthesia for patients with radical mastectomy of breast cancer, which is beneficial to maintain hemodynamic stability, relieve stress reaction and reduce the occurrence of adverse reactions, which is worthy of clinical application.

Anesthetic effect of Dexmetomidine combined with Sevoflurane inhalation in hysteroscopic electric resection

Objective To investigate the anesthetic effect of Dexmetomidine combined with Sevoflurane inhalation in hysteroscopic electric resection. Methods From April 2016 to April 2018, 98 patients received hysteroscopic electric resection treatment in Xuzhou City Hospital of TCM were taken as the study subjects, and they were divided into observation group and control group according to the random number table method, with 49 cases in each group. Patients in the control group were given Sevoflurane inhalation anesthesia intervention, while patients in the observation group were given Dexmetomidine intravenous injection on the basis of the control group. The levels of the hemodynamic indexes before the start of anesthesia (T0), intraoperative (T1), postoperative wakefulness (T2), 10 min after wakefulness (T3) of two groups were compared. The sedation scores (Ramsay), alert/calm observed score (OAA/S), harem shrinkage pain, and intraoperative adverse reactions of two groups were observed after operation. Results The heart rate of the observation group was lower than the control group at T1, T2 and T3, and the differences were statistically significant (all P < 0.05). The scores of Ramsay and OAA/S in the observation group were higher than those in the control group, and the differences were statistically significant (all P < 0.01). The excellent and good rate of postoperative hysterectomy in the observation group was higher than that in the control group, and the difference was statistically significant (P < 0.05). The incidence of respiratory inhibition in the observation group was lower than the control group, the difference was statistically significant (P < 0.05). Conclusion Dexmetomidine intravenous injection combined with Sevoflurane inhalation anesthesia in hysteroscopic electric resection has significant effect of anesthesia, advantageous to maintain patients′ hemodynamic stability, reduce the risk of adverse reactions, and with good security, which is worthy of clinical popularization and application.

Comparison of isobaric and hyperbaric Bupivacaine when used for spinal anesthesia in cesarean section

Objective To compare isobaric and hyperbaric Bupivacaine when used for spinal anesthesia in cesarean section. Methods Twenty-eight patients in Nanjing First Hospital from January to April in 2018 scheduled to receive cesarean section under spinal anesthesia were randomly allocated into two groups: group-I (isobaric Bupivacaine group) and group-H (hyperbaric Bupivacaine group) by random number table method, with fourteen cases in each group. Isobaric 0.5% Bupivacaine (0.75% Bupivacaine 2 mL diluted by cerebrospinal fluid 1 mL) 10 mg and hyperbaric 0.5% Bupivacaine (0.75% Bupivacaine 2 mL diluted by 10%GS 1 mL) 10 mg were administered for patients in group-I and group-H respectively. The mean arterial pressure (MAP), heart rate (HR), numbness level and modified Bromage score (MBS) were recorded at four time points: 1 min (T1), 5 min (T2), 10 min (T3) and 20 min (T4) after anesthesia. Hypotension and the number of phenylephrine use were recorded. Nausea and vomiting, the first time point that the ST segment began to change and the time interval between anesthesia and surgery (Ts) were also recorded. Results The numbness level was lower and MBS was higher in group-I than those in group-H significantly (P < 0.05). The HR at T1 and T3 were lower in group-I than those in group-H significantly (P < 0.05). The incidence of hypotension and number of phenylephrine use were less in group-I than those in group-H significantly (P < 0.05). The incidence of nausea was lower and the first time that the ST segment began to change was later in group-I than those in group-H significantly (P < 0.05). Conclusion Compared with hyperbaric Bupivacaine, isobaric Bupivacaine has longer onset time, lower incidence and degree of hypotension and less side effects when used for spinal anesthesia in cesarean section.

Effects of Bushen Jiangu Decoction on postoperative healing and lumbar function in patients with osteoporotic compression fracture

Objective To investigate the effect of Bushen Jiangu Decoction on postoperative healing and lumbar function in patients with osteoporotic compression fracture (OVCF). Methods A total of 93 cases of OVCF admitted to Panjin Central Hospital from January 2016 to January 2018 were selected as study subjects, and according to the random number table method, they were divided into control group (n = 46) and study group (n = 47). The patients in the control group were treated with Vitamin D Calcium Chewable Tablets, while the patients in the study group were treated with Bushen Jiangu Decoction on the basis of the treatment in the control group. They were treated for 14 days. Total clinical effective rate of the two groups after treatment was counted. The visual analogue scale (VAS) and Japanese Orthopedic Association low back pain scale (JOA) scores, bone mineral density of lumbar spine L2-L4, trochanter of femur, femur and the wedge angle of the injured vertebra, the Cobb angle of kyphosis and ratio of anterior vertebral height of injured vertebrae were measured before and after treatment. The adverse reactions of the two groups were observed. Results The total clinical effective rate of the study group was significantly higher than that of the control group, and the difference was statistically significant (P < 0.05). After treatment, the VAS score , the wedge angle of the injured vertebra, the Cobb angle of kyphosis of the two groups were smaller than those before treatment, and those in the study group were less than the control group (P < 0.05), while the JOA score, bone mineral density of lumbar spine L2-L4, trochanter of femur, neck of femur, and ratio of anterior vertebral height of injured vertebrae were significantly higher than those before treatment, and those in the study group were better than the control group (P < 0.05). No serious adverse reactions occurred in the two groups during treatment. Conclusion The clinical curative effect of Bushen Jiangu Decoction in the treatment of OVCF is significant, which can significantly improve pain and clinical symptoms, promote postoperative fracture healing and improve lumbar function. It has high security, that provides a new thinking for clinical treatment of OVCF.

Effect of Tangshiming Capsules on visual acuity and quality of life in patients with diabetic retinopathy in TCM syndrome type of qi-yin deficiency and blood stasis

Objective To observe the effect of Tangshiming Capsules on the visual acuity and quality of life in patients with diabetic retinopathy (DR) in TCM syndrome type of qi-yin deficiency and blood stasis. Methods Ninety two (184 eyes) DR cases admitted to the Second Affiliated Hospital of Shandong University of Traditional Chinese Medicine from February to August 2017 were selected from Ward and Outpatient of Departments of Endocrinology and Ophthalmology. They were divided into experimental group and control group by random number table method with 45 cases (90 eyes) in the experimental group and 47 cases in the control group (94 eyes). Eventually, eighty DR cases finished follow-up, with 40 cases (80 eyes) in each group. The control group was given the treatment of control blood pressure, blood glucose, blood lipids and lifestyle intervention. The experimental group was given Tangshiming Capsules orally on the basis of the treatment of the control group. Quality of life, visual acuity, TCM syndrome scores, fasting blood glucose, 2 h postprandial blood glucose, glycosylated hemoglobin changes and safety evaluation indicators (blood routine, urine routine, liver and kidney function, electrocardiogram) were observed before and after treatment. Results Before treatment, there was no significant difference in the scores of quality of life between the two groups (P > 0.05), which were all significantly improved after treatment (P < 0.05), and the improvement in the experimental group was superior than the control group (P < 0.05). Before treatment, there was no significant difference in visual acuity between the two groups (P > 0.05), which both improved in the two groups after treatment (P < 0.05), and the improvement in the experimental group was superior to the control group (P < 0.05). Before treatment, there was no significant difference in the syndrome scores between the two groups (P > 0.05), which both reduced after treatment in the two groups (P < 0.05), and the scores was lower in the experimental group than the control group (P < 0.05). There was no significant difference in glycosylated hemoglobin, fasting blood glucose, and postprandial blood glucose 2 h before treatment between the two groups (P > 0.05), which all reduced significantly after treatment (P < 0.05), and those in the experimental group was lower than the control group (P < 0.05). The total effective rate was 82.50% in the experimental group, which was higher than 55.00% in the control group (P < 0.05). Blood and urine routine, liver and kidney function were normal in both groups before and after treatment, and no adverse reactions occurred. Conclusion Tangshiming Capsules can significantly improve the visual acuity and quality of life of DR patients, and the security was good.

Efficacy of free-frying Chinese medicine granules as replacement of Chinese herbal medicine in treating hypertension:a Meta analysis

Objective To systematically review the efficacy of free-frying Chinese medicine granules as replacement of Chinese herbal medicine in treating hypertension, and to provide the evidence for clinical treatment and the selection of therapy. Methods A systematic search by online databases was performed, including CNKI, WanFang, VIP and PubMed, searching period from the inception to December 3rd, 2017. Randomized controlled trials (RCTs) of free-frying Chinese medicine granules vs. Chinese herbal medicine were collected. According to the Cochrane Handbook, data of the articles conforming to inclusion criteria was extracted and the quality of the articles was evaluated, The meta analysis was performed via RevMan 5.2 software. Results A total of 8 trails were enrolled, which included 1433 cases. Meta analysis results showed that there was no significant difference in efficiency, blood pressure target achievement ratio and TCM symptom score between the treatment of free-frying Chinese medicine granules and Chinese herbal medicine (P > 0.05). Conclusion There is no significant difference between and Chinese herbal medicine in the treating efficacy of hypertension. In the selection of clinical therapy, the minimum cost method in pharmacoeconomics can be the reference for selection, and the treating plan with the minimum cost advantage is the optimal one.

Effect of Bupi Yishen Huoxue Decoction on patients with renal anemia in the TCM symptom type of deficiency of spleen and kidney and blood stasis

Objective To observe the effect of Bupi Yishen Huoxue Decoction on patients with renal anemia in the TCM symptom type of deficiency of spleen and kidney and blood stasis. Methods Sixty cases with renal anemia admitted to Department of Chinese Medicine, Ningde City Hospital Affiliated of FuJian Medical University from May 2017 to March 2018 were selected and they were divided into treatment group and control group, with 30 cases in each group by random number table method. The control group was given basic treatment, and the treatment group was given the Bupi Yishen Huoxue Decoction based on the treatment in the control group. The two groups were treated for 2 months. Clinical syndrome score, red blood cell (RBC), Haemoglobin (Hb), hematocrit value (HCT), blood urea nitrogen (BUN) and serum creatinine (Scr) were observed and compared between the two groups. Results After treatment, the clinical syndrome scores of the two groups were lower than those before treatment, and the differences were statistically significant (P < 0.05), the score in the treatment group was lower than that in the control group (P < 0.05). After treatment, RBC, Hb, HCT in the two groups were higher than those before treatment, the differences were significant (P < 0.05), meanwhile the levels in the treatment group were higher than those in the control group (P < 0.05). After treatment, BUN and Scr in the two groups all decreased compared to those before treatment (P < 0.05), meanwhile the levels in the treatment group were lower than those in the control group (P < 0.05). There were no obvious adverse reactions in the two groups. Conclusion Bupi Yishen Huoxue Decoction can effectively improve renal anemia and renal function, which is worth of promotion.

Professor ZHANG Peiqing is a chief physician of Heilongjiang Academy of Traditional Chinese Medicine, a national famous Chinese medicine expert. Professor Zhang has worked in clinic for dozens of years, that has accumulated rich clinical experiences in TCM internal medicine diseases, difficult and complicated cases, especially cardiovascular diseases. This article introduces Professor ZHANG Peiqing′s academic thought on the treatment of coronary heart diseases, including the understanding of the etiology and pathogenesis of coronary heart disease, which is the heart and vein deficiency caused by internal invasion of cold, abnormal emotion, improper diet, deficiency dueto by aged and excessiveness. Professor Zhang proposes four principles in the treatment of coronary heart disease. This article summerizes the experience of Professor Zhang′s clinical experience, that syndrome differentiation and treatment of deficiency syndrome should be distinguished by qi, blood, Yin, Yang and deficiency and excess. In the treatment, combination of dredging and tonifying method should be used concurrently, and the excess should be treated in distinction from blood stasis to phlegm, that the clinical effect was satisfactory.

Analysis on the differential diagnosis and imaging features of rare desmoid-type fibromatosis

Objective To explore the imaging manifestation of rare desmoid-type fibromatosis (DF), in order to further improve the diagnostic accuracy of the disease. Methods The clinical and imaging findings of 8 patients with DF confirmed by surgery and pathology from July 2015 to October 2017 in the First Affiliated Hospital of University of Science and Technology of China Anhui Provincial Hospital were retrospectively analyzed. The location and boundary of the lesion were analyzed, and the chayacteristics and differential diagnosis of computed tomography (CT) and magnetic resonance (MR) imaging were analyzed. Results In the 8 patients, 5 cases were abdominal wall type, and 3 cases were intra-abdominal type. Seven cases had a history of abdominal surgery in the 8 cases. The abdominal wall showed infiltrative growth, and it was not clear with the rectus abdominals muscle. The spindle shape was the most common form, and the growth axis was parallel to the muscle fiber. The intra-abdominal type was mainly inflated growth, with varied morphology and clear boundary. CT scan: the lesions showed soft tissue density, low density area could be seen, with rare calcification and hemorrhage, boundary was less clear. MR scan: T1WI showed equal or slightly low signal, T2WI showed a slightly uneven high signal, low signal area could be seen; enhanced scanning showed progressive enhancement. Conclusion Abdominal wall type and intra-abdominal type desmoid fibromatosis have certain characteristics in clinical data, CT and MR imaging, which is of great value for the correct diagnosis of this disease.

The clinical application value of ultrasonography in arteriovenous fistula of end stage renal disease

Objective To investigate the value of ultrasonography in preoperative evaluation and postoperative dynamic monitoring of vascular access in patients with end stage renal disease have arteriovenous fistula. Methods Eighty-four ESRD patients with AVF admitted to Jiangjin District Central Hospital of Chongqing from January 2016 to November 2017 were selected. Preoperative evaluation and postoperative detection of artificial arteriovenous vascular access were performed by ultrasonography. Results In 84 cases of ESRD patients who were preambulatory AVF, 5 cases were found unsuitable for AVF by ultrasonography, and were selected other pathways for dialysis. In the remaining 79 patients underwent AVF, ultrasonography revealed that 16 patients failed to achieve fistula due to various postoperative complications, and guided clinical interventions. Conclusion Ultrasonography can accurately guide clinicians to select appropriate arteriovenous blood vessels and improve the success rate of fistula operation in ESRD patients undergoing AVF preoperative artificial arteriovenous vascular access function assessment. In addition, dynamic monitoring of arteriovenous internal fistula function, complications and hemodynamic status after operation is of great significance in guiding further clinical treatment.

Diagnostic value of MR quantitative measurement indexes in patellar instability in adolescents

Objective To analyze the diagnostic value of magnetic resonance (MR) quantitative measurement indexes in patellar instability in adolescents. Methods Forty-eight cases (48 knees) of adolescent patients with patellar instability (case group) treated in Department of Orthopedics of the Second Affiliated Hospital of Xinjiang Medical University and 30 cases of healthy young volunteers for health check-ups and recruitment (control group) from June 2016 to May 2018 were selected as research objects. They were given MR plain scan for knee joint. Related quantitative indexes were measured on cross-sectional fat-reducing FSE PDWI, including patellar deviation index, patellar tilt angle, patellar external displacement, trochlear groove angle, lateral femoral trochlear tilt angle, and tibial tuberosity-trochlear groove distance (TT-TG). And the receiver operating curve (ROC) was used to analyze the diagnostic efficacy of indexes for patellofemoral joint instability. Results Compared with control group, the patellar deviation index, patellar tilt angle, patellar external displacement, trochlear groove angle and TT-TG in case group were significantly larger, and the lateral femoral trochlear tilt angle was significantly smaller, the differences were statistically significant (P < 0.05). The patellar deviation index, patellar tilt angle, patellar external displacement and trochlear groove angle of female adolescent patients with patellar instability were lager than those of male adolescent patients with patellar instability, the differences were statistically significant (P < 0.05). And there was no significant difference between male and female adolescents with patellar instability in lateral femoral trochlear tilt angle and TT-TG (P < 0.05). Among MR quantitative measurement indexes, the areas under the ROC curve of patellar deviation index, patellar tilt angle, patellar external displacement, trochlear groove angle, lateral femoral trochlear tilt angle and TT-TG were all>0.9, and the sensitivity and specificity of diagnosing patellar instability in adolescents were all>85%. Conclusion In the state of knee extension, MR quantitative measurement of patellofemoral joint indexes have a high diagnostic efficiency for patellar instability in adolescents. There are gender differences in the patellar deviation index, patellar tilt angle, patellar external displacement and trochlear groove angle, which has a certain guiding role in the assessment of patellar instability in adolescents with different gender.

The diagnostic value of brain 18F-FDG PET/CT in the localization and lateralization of brain epileptogenic focus

Objective To explore the value of fludeoxyglucose (18F-FDG) positron emission tomography-computed tomography (PET/CT) in the accurate localization and lateralization of brain epileptogenic focus. Methods From June 2011 to July 2013, 69 patients with medically intractable epilepsy from the General Hospital of Northern Theater Command were selected. 18F-FDG interictal PET/CT, magnetic resonance imaging (MRI), electroencephalo gram (EEG) were performed in those patients and the results were compared with the electroenephalogram monitoring during operations and the pathological tests. Results Among 69 cases of epileptic focus resection, accuracy rate of localization and lateralization of epileptogenic focus by interictal 18F-FDG PET/CT were 62.3% (43/69) and 78.3% (54/69) respectively. Among 43 case of PET/CT localization of simple temporal lobe lesions, the intraoperative electroencephalogram monitoring and surgical pathology confirmed the same localization in 36 cases and the same lateralization in 37 cases, with an accuracy rate of 83.7% (36/43) and an accuracy rate of 86.0% (37/43), which were higher than those in non-temporal lobe lesions [26.9% (7/26), 65.4% (17/26)], the differences were statistically significant (P < 0.05). The lateral accuracy [78.3% (54/69)] of PET/CT for brain epilepsy was significantly higher than that in the location [62.3% (43/69)], the difference was statistically significant (P < 0.05). Accuracy rate of localization of epileptogenic focus in 69 patients by MRI was 42.0% (29/69), accuracy rate of localization of epileptogenic focus in 69 patients by EEG were 44.9% (31/69). PET/CT had higher accuracy rate of localization than MRI and EEG (χ2 = 5.692, 4.196, all P < 0.05). Conclusion Interical 18F-FDG PET/CT possesses higher value than routine examinations such as MRI and EEG in the localization of epileptogenic focus, especially in temporal lobe localization focus.

The myocardial protective effect of Danshen Dripping Pills in patients with ST-segment elevation myocardial infarction after percutaneous coronary intervention

Objective To study the protective effect of Danshen Dripping Pills in patients with ST-segment elevation myocardial infarction (STEMI) after percutaneous coronary intervention (PCI). Methods A total of 109 patients with STEMI in Xianyang Central Hospital of Shaanxi Province from January 2014 to March 2016 were enrolled in this study. The subjects were randomly divided into the control group (54 cases) and the treatment group (55 cases) by the random number table method. All the patients were treated with PCI, and then the control group was treated with Atorvastatin Calcium, the treatment group was treated with Danshen Dripping Pills on basis of the control group. The two groups were treated for 3 months. The levels of serum cardiac troponin I (cTnI), creatine kinase isoenzyme (CK-MB), brain natriuretic peptide (BNP), interleukin 6 (IL-6), C-reactive protein (CRP), tumor necrosis factor-α (TNF-α), nitric oxide (NO), endothelin (ET) and soluble adhesion molecules-1 (sICAM-1) of the two groups before and after treatment were compared and detected. The adverse cardiovascular events of the two groups after discharge for 2 years were compared. Results After treatment, the levels of serum cTnI, CK-MB and BNP of the control group were significantly higher than before treatment, the levels of serum cTnI, CK-MB and BNP of the treatment group were significantly lower than before treatment, and the levels of serum cTnI, CK-MB and BNP of the treatment group were significantly lower than the control group (P < 0.05). After treatment, the levels of serum IL-6, CRP, TNF-α, ET and sICAM-1 of the two groups were significantly lower than before treatment, the serum NO of the two groups was significantly higher than before treatment, and the level changes of serum IL-6, CRP, TNF-α, ET, sICAM-1 and NO in treatment group were better than those of the control group (P < 0.05). There was no significant difference of the rates of adverse cardiovascular events of the two groups during the follow-up period of 2 years (P > 0.05). Conclusion Based on the application of statins, Danshen Dripping Pills in STEMI patients after PCI has strong cardioprotective effect. It can reduce the inflammatory reactions and vascular endothelial injury induced by surgery, with good prognosis, and it is worthy of clinical application.

Preventive effect of Low Molecular Weight Heparin on preoperative deep vein thrombosis in elderly patients with intertrochanteric fracture

Objective To investigate the preventive effect of Low Molecular Weight Heparin on preoperative deep vein thrombosis in elderly patients with intertrochanteric fracture. Methods Fifty-two elderly patients with intertrochanterteric fracture in accordance with the inclusion criteria admitted to Jizhong Energy Fengfeng Group General Hospital from August 2015 to July 2018 were selected as the experimental group, they were given Low Molecular Weight Heparin to prevent preoperative deep venous thrombosis after admission. 45 elderly patients with intertrochanterteric fracture without using Low Molecular Weight Heparin to prevent preoperative deep venous thrombosis from December 2012 to July 2014 were selected as the control group. The incidence of preoperative deep venous thrombosis in the two groups was calculated. SPSS 21.0 sofyware was used to analyze the incidence of preoperative deep venous thrombosis and the type distribution of deep venous thrombosis between the two groups. Results The incidence of preoperative deep venous thrombosis had no significant difference between the experimental group [13.5% (7/52)] and the control group [24.4% (11/45)] (P = 0.165). There was no significant difference in the type distribution of deep vein thrombosis between the two groups (P = 1.000). Conclusion The effect of using only Low Molecular Weight Heparin in prevention of deep vein thrombosis in elderly patients with intertrochanteric fracture may not be obvious. Other physical methods still need to be combined for prevention.

The Clostridium difficile infection conditions and molecular epidemic characteristics of clinical diarrhea patients

Objective To understand the infection conditions and molecular epidemic characteristics of Clostridium difficile in clinical patients with diarrhea, to provide a basis for controlling nosocomial infections and clinical diagnosis and treatment. Methods Eight hundred and fifty-two feces samples from clinical diarrhea patients in the Second Hospital of Jiaxing City from January to December 2016 were collected as experimental group and 310 feces samples from healthy examination adults in the same period were collected as control group. The positive rate of Clostridium difficile was obtained by anaerobic culture and taken comprehensive analysis by combining with toxin genotype and multilocus sequence typing. Results A total of 108 Clostridium difficile were isolated from 852 feces samples of experiment group, with a positive rate of 12.68%, while a total of 8 Clostridium difficile were isolated from 310 feces samples of control group, with a positive rate of 2.58%, the difference between the two groups was statistically significant (P < 0.05). The experiment group was dominated by tcdA+tcdB+ strains, accounted for 86.11% (93/108), tcdA-tcdB+ strains accounted for 5.56% (6/108), and tcdA-tcdB-strains accounted for 8.33% (9/108), the rate of toxin production was 91.67%, while the corresponding toxin genotypes of the control group accounted for 62.50% (5/8), 12.50% (1/8), 25.00% (2/8), and the rate of toxin production was 75.00%, the rate of toxin production between the two groups had no statistically significant difference (P > 0.05). In the experiment group, 32 ST types were found by multisite sequence typing, among which, ST54 was a relatively popular type, while CC48 was a relatively popular clonal complex. Conclusion The infection rate of Clostridium difficile in clinical patients with diarrhea is high, which is urgent to attract clinical attention and strengthen the prevention and control of the hospital infection, to prevent epidemic outbreak.

Effects of animation education on compliance of respiratory function exercise and self management efficacy for patients with lung tumor perioperative period

Objective To explore the influence of animation education on the compliance of respiratory function exercise and self-efficacy in perioperative lung tumor patients. Methods From September 2015 to March 2016, in Xiangya Hospital of Central South University, the Second Xiangya Hospital of Central South University, the Third Xiangya Hospital of Central South University and Hu′nan Cancer Hospital, 80 perioperative lung tumor patients were selected. By the random number table method, they were divided into intervention group and control group, with 40 cases in each group. The control group adopted routine health education, and the intervention group adopted the self-designed animation of respiratory function exercise on the basis of the control group. The levels of respiratory function exercise compliance and self-management efficacy in the two groups before and after the intervention were compared. Results After intervention, the score of respiratory function exercise compliance, total score and each dimension score of self-management efficacy in the intervention group were significantly higher than those in the control group, the differences were statistically significant (P < 0.05 or P < 0.01). Conclusion The animation education method can improve perioperative lung tumor patients′ compliance of respiratory function exercise and self-management efficacy.

Effect of clinical pathway nursing on the postoperative nutritional status-related complications and quality of life of patients with cholenterostomy

Objective To explore the effect of clinical pathway nursing on the postoperative nutritional status-related complications and quality of life in patients with cholenterostomy. Methods From April 2015 to April 2017, in the First Affiliated Hospital of Xinjiang Medical University, 70 patients with cholenterostomy were selected. By random number table method, they were divided into control group and observation group, with 35 cases in each group. The control group was given operating routine nursing care, observation group was given the clinical pathway nursing, postoperative nutritional status-related complications and quality of life in two groups were compared. Results The rate of gastrointestinal tract reaction, metabolic complications (including hyperglycemia reaction, hypoglycemia reaction), abnormal liver function in observation group were lower than those in control group, the differences were statistically significant (P < 0.05). After operation, the body function, psychological function, social function and material life grade rise in two groups were higher than those before operation, which in observation group were higher than those in control group, the differences were statistically significant (P < 0.05). Conclusion The clinical pathway nursing is more conducive to decrease the rate of postoperative nutritional status-related complications of patients with cholenterostomy and improve the quality of life after surgery.

Nursing effect of quality control circle combined with clinical nursing pathway in patients with lumbar degenerative changes

Objective To evaluate the nursing effect of quality control circle combined with clinical nursing pathway in patients with lumbar degenerative changes. Methods From April 2016 to April 2017, in the First Affiliated Hospital of Liaoning University of Traditional Chinese Medicine, 120 patients with lumbar degenerative changes were selected, the 60 cases given traditional nursing were as the control group, and the other 60 cases given quality control circle combined with clinical nursing pathway on the basis of the control group were as the observation group. The nursing effect, Oswestry disability index (ODI) score and satisfaction in the two groups were compared. Results The pain scores of lower limbs and waist in the observation group [(1.20±0.06), (1.12±0.03) points] were lower than those in the control group [(3.68±1.50), (3.52±1.36) points], the differences were statistically significant (P < 0.05); the ODI scores in the observation group at discharge and three months after discharge [(25.18±10.15), (16.10±10.04) points] were lower than those in the control group [(31.22±11.20), (24.19±10.16) points], the differences were statistically significant (P < 0.05); the total nursing satisfaction rate in the observation group (98.33%) was higher than that in the control group (83.33%), the difference was statistically significant (P < 0.05). Conclusion For patients with lumbar degenerative changes, quality control circle combined with clinical nursing pathway can alleviate their pains, reduce their lumbar dysfunction, and improve their nursing satisfaction.
