42 Research progress on SIRT5 as deacylase
ZHENG Chunlei1,2 LU Wenqing1,2 CHE Xiaofang1,2 LIU Yunpeng1,2▲
Sirtuin 5, also called SIRT5, is a member of Sirtuins family. Besides its deacetylation activity, SIRT5 also has strong desuccinylation, demalonylation and deglutarylation activities. Therefore, SIRT5 has been attracted much attention in recent years. Currently, about more than tenproteins were identified as substrates of SIRT5. SIRT5 is involved in the regulation of glucose oxidation, ROS metabolism and fatty acid oxidation, ammonia removal, detoxification, anti apoptosis, inflammation and so on, by playing the same enzymatic activity on different substrates, or playing different enzymatic activity on the same substrate. The abnormal expression of SIRT5 is closely related to the occurrence and development of tumor, though its function is not clarified yet. SIRT5 can play the role of oncogene to promote tumor proliferation, metastasis, drug resistance and metabolic reprogramming. On the other hand, SIRT5 also play the role of tumor suppressor gene to inhibit tumor cell growth and apoptosis. In addition, SIRT5 abnormalities are also closely related to hypertrophic cardiomyopathy, myocardial infarction and other cardiovascular diseases, such as Parkinson′s disease and Alzheimer′s disease. In order to have a good comprehensive on the research progress of SIRT5, this paper reviews the structure and regulation, enzyme activity and biological function of SIRT5, and the relationship between SIRT5 and desease in details here.

46 Strategies for immunotherapeutic targeting of regulatory T cells in tumor microenvironment
LIU Jialing1* WANG Shimin1* ZHUO Zhaozhen2 YUAN Jun3
Regulatory T cell (Treg) is a group of lymphocytes with negetive regulation of inhibiting tumor-associated antigen T cell-mediated immune responses, targeted agents may influence tumor immunotherapy and vaccine activation. Systemic removal of CD25+CD4+ T cells perhaps elicit tumor growth in rodents, but also increasing the severity of the autoimmune diseases as peripheral self-tolerance is maintained by Treg cells. According to sources of Regulatory T cells, it can be classfied as two population of nTreg and iTreg cells. Regulatory T cells are phenotypically diversity and functionally heterogeneous in different types of tumor and tissues, whereas conferring the same marker of CD4, CD25 and foxp3. Because of its tumor heterogeneity and organ heterogeneity, therefore, the strategy of targeting Treg cells in tumor immunotherapy is different. On the other hand, removal Treg cell strategies is different in tumor immunotherapy, it can remove regulatory T cells (Treg) or dampen the function of Treg cells through limit its immunosuppression protein, such as GITR, T-bet, Neoropilin-1, CTLA-4, IDO or PD-1. The autoimmunities can be prevented by inoculating OVA vaccine when removing Tregs .Ipilimumab, which has recently been FDA-approved for therapy patients with advanced melanoma mechnism is targeting CTLA- 4 in Treg cells. In this paper, we review the phenotype, heterogeneity and tumor therapy strategies of Treg treatment in order to avoid immune disorders in the future.

51 The interaction of Clopidogrel with SSRIs mediated by CYP2C19
FAN Youmin1 MA Xiuping2 CHEN Songjie1 GENG Deqin2▲
Along with the general rising of the incidence of post-stroke depression, selective serotonin reuptake inhibitors (SSRI) and dual anti-platelet therapy are widely used for patients with post-stroke depression in clinic. However, both SSRI and Clopidogrel are metabolized by hepatocyte pigment P450 isoenzyme system, some meta-analysis and small sample researches indicate that SSRI can reduce the anti-platelet effects of Clopidogrel by inhibiting the activity of hepatocyte pigment P450 isoenzyme CYP2C19. According to the different chemical structures and pharmacokinetic characteristics of SSRIs, the degrees of suppression of SSRI are not alike. This article summaries the drug metabolism pathway, mechanism and clinical research progress of this two drugs.

ZHANG Bingyi1 GUO Ruiqiang2▲ ZHOU Qing2 PING Jie1 ZHANG Ling1 ZHANG Xiaofeng1
Objective To assess the immediate impact of preload reduction caused by single hemodialysis on left ventricular longitudinal systolic function in uremic patients with real-time three plane (real-time three-plane, RT-3PE) automatic functional imaging (AFI) technology. Methods Thirty-one patients with uremia were selected who were underwent routine hemodialysis in People′s Hospital of China Three Gorges University and Renmin Hospital of Wuhan University from December 2013 to March 2017. Left ventricular peak systolic longitudinal strain (PSLS) and bovine eye diagram were detected by using RT-3PE AFI technology before and after routine hemodialysis immediately. The change of indicators was compared before and after routine hemodialysis. Results Compared with pre-hemodialysis, post- hemodialysis the LVEF values had no statistically significant changed (P > 0.05), the average values of left ventricular whole PSLS were significantly reduced (P < 0.05). Segmental analysis showed that, the values of local PSLS only appeared in the middle period decreased significantly after hemodialysis (P < 0.01). There was no significant change in the apex section and basal segment (average P > 0.05). After hemodialysis, left ventricular overall PSLS and LVEF was significantly negative correlated (r = -0.78, P < 0.01), and Sm also showed significant negative correlation (r = -0.50, P < 0.50). While left ventricular overall PSLS and dialysate volume was significantly positive correlated (r = 0.63, P < 0.05). Conclusion As showing by the results of PSLS measured by AFI analysis, application of left ventricular PSLS can accurately evaluate left ventricular longitudinal contraction function in patients with uremia. Under the condition of no changing the whole systolic function of left ventricular, single dialysis can affect the left ventricular longitudinal systolic function by reducing preload, significant effects change only appeared in the middle section. When using PSLS assessment of left ventricular systolic function in uremia patients, doctors should pay attention to the state of preload.
