5 Study on mechanism of action of Galangin cream against vitiligo based on network pharmacology and experimental verification

Objective To investigate the mechanism of action of Galangin (GA) cream against vitiligo by network pharmacology and animal experiments. Methods The active ingredients and targets of GA were screened through Swiss Target Prediction, Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform, and vitiligo-related targets were screened in TTD, OMIM, and DrugBank databases. The drugs and disease targets were imported into the String platform to construct a protein-protein interaction (PPI) network to screen the core targets. Cytoscape 3.7.1 software was used to construct the network of traditional se medication-active ingredients-core target-key pathways, and enrichment analysis was performed. The molecular docking of GA with the core target protein was performed by Autodock Vina 1.2. Sixty SPF male C57BL/6 mice aged four weeks, weighing (21±3) g, were selected, of which ten mice were set as the normal control group, and the other 50 mice were evenly applied with 10%H2O2 on the back to establish the vitiligo model. After successful modeling, the mice were divided into model group, positive drug group (8-methoxyepsoralen, 4.25 mg/kg), and GA cream high (4%GA, 1g/ time), medium (2%GA, 1g/ time), low (1%GA, 1g/ time) dose groups, with ten rats in each group. After 30 days of continuous administration, the changes of mice skin tissues were observed by hematoxylin-eosin staining; the serum levels of interleukin-18 (IL-18), tumor necrosis factor-α (TNF-α), and endothelin-1 (ET-1) of mice were detected by enzyme-linked immunosorbent assay. The protein expression levels of TNF-α and ET-1 in the skin tissues of mice were detected by Western blot. The mRNA expression of ET-1, IL-18, and TNF-α in the skin tissues of mice was detected by real-time fluorescent quantitative PCR. Results A total of 17 GA targets, 459 vitiligo targets, and ten drug-disease targets were identified. PPI network showed that tyroinase, TNF, IL-1β, and IL-18 were the core targets of GA in the treatment of vitiligo. Enrichment analysis showed that GA may play an anti-vitiligo effect through nuclear factor-κB signaling pathway, NOD-like receptor signaling pathway, and HIF-1 pathway. Molecular docking results showed that GA had good binding ability with ET-1, TNF-α, and IL-18. The results of hematoxylin-eosin staining showed that there was no obvious abnormality in the normal control group. In the model group, the fold of the epidermal spinous layer became thicker, the hair follicle was damaged, and the stratum corneum was significantly proliferated. There were slight spinous folds in the epidermis of the GA cream groups, which were lighter than those of the model group. The serum levels of ET-1, IL-18, and TNF-α in the model group were higher than those in the normal control group, the protein expressions of ET-1 and TNF-α in the skin tissues were higher than those in the normal control group, and the mRNA expressions of ET-1, IL-18, and TNF-α in the skin tissues were higher than those in the normal control group (P<0.05). The levels of serum ET-1, IL-18, and TNF-α in GA cream low, medium, high dose groups were lower than those in the model group, the expression of ET-1 protein in the skin tissues in the GA cream high dose group was lower than that in the model group, the expression of TNF-α protein and IL-18 mRNA in the skin tissues in the GA cream medium and high dose groups were lower than those in the model group, the mRNA expressions of ET-1 and TNF-α in the skin tissues of the GA cream low, medium, high dose groups were lower than those of the model group (P<0.05). The expression of TNF-α protein and IL-18 mRNA in skin tissues of GA cream medium and high dose groups were lower than those of GA cream low dose group, and the expression of ET-1 protein in skin tissues of GA cream high dose group was lower than that of GA cream low dose group (P<0.05). Conclusion GA cream may exert its anti-vitiligo effect by regulating various pathways and inhibiting ET-1, IL-18, and TNF-α.

12 Study on the medication rules of Categorized Collection of Medical Formulas in treatment of stomach stuffiness disease

Objective To study and summarize the medication rules of the prescription included in Categorized Collection of Medical Formulas for the treatment of stomach stuffiness disease. Methods Excel 2016, IBM SPSS Modeler 18.0, and IBM SPSS Statistics 25 were used to make statistics on the frequency of drugs contained in the prescriptions for treating stomach stuffiness disease in the “fivezang men”, “zhuqi men”, “piwei men”, and “zhangman men” in the Categorized Collection of Medical Formulas. The efficacy, four natures, five flavours, distribution of channel tropism and use proportion of aromatic drugs were observed. Apriori association rule analysis and network analysis were performed for all drugs, and systematic cluster analysis was performed for high-frequency drugs. Results Categorized Collection of Medical Formulas was screened 426 prescriptions for the diagnosis of stomach stuffiness disease, involving 207 kinds of drugs, mainly to regulating qi-flowing drugs and tonifying deficiency drugs to treat stomach stuffiness disease, of which regulating qi-flowing drugs mainly to Citri Reticulatae Pericarpium, Aucklandiae Radix, tonifying deficiency drugs mainly to Glycyrrhizae Radix Et Rhizoma, Ginseng Radix Et Rhizoma, Atractylodis Macrocephalae Rhizoma. The four nature and five flavour of drugs were mainly pungent, bitter, and warm, and the channel tropism of drugs were mainly spleen, stomach, lung, and kidney channels. Aromatic drugs accounted for 32.90% of the total drug frequency. A total of 15 binomial association rules were obtained, and high-frequency drugs could be grouped into nine categories. Conclusion In Categorized Collection of Medical Formulas, the method of regulating qi and tonifying deficiency is the basic principle of treating stomach stuffiness disease, and on this basis, the method of warming, eliminating phlegm (dampness) and eliminating food is used.

18 Effects of LAMC1 gene silencing on proliferation and cell cycle of bladder cancer cells

Objective To investigate the expression of recombinant laminin gamma 1 (LAMC1) in bladder cancer cells and the effect of silencing lamc1 gene expression on the proliferation and cell cycle of bladder cancer cells. Methods Cancer and adjacent tissues of 48 patients with bladder cancer admitted to Handan Central Hospital, Hebei Province from January 2019 to December 2021 were selected. RT-qPCR was used to detect the expression of LAMC1 mRNA in UMUC3, T24, 5637, and J82 cells. LAMC1-siRNA plasmid was transfected into 5637 cells, LAMC1 silencing group and negative control group were set up, and the transfection efficiency was verified by RT-qPCR and Western blot. CCK-8 assay, clonogenesis assay and flow cytometry were used to detect the effect of LAMC1 gene silencing on the proliferation, clonogenesis ability, and cell cycle of bladder cancer cells. Western blot was used to detect the expression levels of cyclin A, cyclin D1, p16, and p21. Results The expression level of LAMC1 mRNA in 5637 cells was higher than that in UMUC3, T24, and J82 cells (P<0.05). There was no significant difference in LAMC1 positive expression rate between bladder and paracancer tissues (P>0.05). LAMC1 mRNA and protein expression levels in LAMC1 silencing group were lower than those in negative control group (P<0.01). Cell proliferation and clonogenesis in LAMC1 silencing group were lower than those in negative control group (P<0.01). The percentage of S phase cells in LAMC1 silencing group was higher than that in negative control group (P<0.01), and the percentage of G1 phase cells was higher than that in negative control group (P<0.01). The expression levels of cyclin D1 and cyclin A proteins in LAMC1 silencing group were highly lower than those in negative control group (P<0.01), while the expression levels of p16 and p21 proteins were higher than those in negative control group (P<0.01). Conclusion Silencing LAMC1 gene expression can inhibit the expression of cell cycle related proteins, thus inhibiting the proliferation of bladder cancer 5637 cells, suggesting that LAMC1 may be a potential therapeutic target for bladder cancer.

24 Imaging features of patients in sequelae of cerebral infarction complicated with community acquired pneumonia and construction and validation of prognostic risk score model

Objective To analyze the imaging features of patients in sequelae of cerebral infarction complicated with community acquired pneumonia (CAP), and to construct and validate prognostic risk score model. Methods A total of 216 sequelae of cerebral infarction complicated with CAP patients from South Taihu Hospital Affiliated to Huzhou College from January 2018 to March 2022 and the 72nd Military Hospital of Chinese People’s Liberation Army from January 2012 to December 2021 were selected. Using random number table method, 216 patients were divided into modeling group (151 cases) and verification group (65 cases) according to the ratio of 7∶3. Death during hospitalization was tracked and divided into death group and survival group. Based on construction data, univariate and multivariate logistic regression were used to analyze the factors in prognosis of sequelae of cerebral infarction complicated with CAP patients; according to the regression coefficient corresponding to each index in the multi-factor regression equation, the risk score model was established; the performance of the model was evaluated by the receiver operating characteristic curve, and the model was validated by the verification group data. Results The fatality rate of 216 sequelae of cerebral infarction complicated with CAP patients was 20.37%, 19.21% in modeling group and 23.08% in verification group. CT showed that the pulmonary infection involved only one lung in the sequelae of cerebral infarction complicated with CAP in 72.69% and both lungs in 27.31%. CT detected lung consolidation in 12.96% and pleural effusion in 21.30%. There were statistically significant differences in age, time from onset to treatment, duration of cerebral infarction, diabetes, mellitus combined with multidrug-resistant gram-negative bacteria (MDR-GNB) infection, moderate and large pleural effusions, neutrophil to lymphocyte ratio (NLR), and lactic acid between death group and survival group (P<0.05). Duration of cerebral infarction≥5 years (OR=5.284), diabetes mellitus(OR=3.515), combined with MDR-GNB infection (OR=13.821), moderate and large pleural effusion (OR=6.618), NLR>5.85 (OR=8.186), and lactic acid >2.66 mmol/L (OR=4.699) were independent risk factors for affecting prognosis of sequelae of cerebral infarction complicated with CAP patients (P<0.05). The area under the curve of risk score model in construction risk score model in the modeling group and the verification group was 0.804 (95%CI: 0.731-0.864) and 0.821 (95%CI: 0.706-0.905), respectively. ≥5 points was classified as high risk of death. Conclusion The risk score model constructed in this study has a good predictive ability for evaluating the prognosis of sequelae of cerebral infarction complicated with CAP patients.

32 Visual analysis of endometriosis-associated infertility based on CiteSpace and VOSviewer

Objective To analyze the visualization results of endometriosis (EM)-associated infertility based on CiteSpace and VOSviewer. Methods The literature on topics related to EM-associated infertility in the CNKI and Web of Science Core Collection from January 2000 to December 2022 was retrieved. CiteSpace and VOSviewer software were used to realize the visual analysis of the authors, institutions, countries, and keywords. Results This study included 895 literatures, 223 in Chinese with a relatively stable trend and 672 in English with an increasing trend in annual number of publications. There were 43 Chinese core authors, the most published was Cao Yunxia; 42 English core authors, the most published was Barbosa Caio P. The institutions that published the most articles in both English and Chinese were Peking Union Medical College Hospital and Sao Paulo University. The country with the most English literature published was the United States. A total of 235 keywords were included in Chinese literature, and the top three keywords were assisted reproductive technology, pregnancy, and laparoscopic surgery, with seven clusters generated. A total of 487 keywords were included in the English literature. In addition to the subject word, the top three keywords were in vitro fertilization, gene-expression, peritoneal fluid. with seven clusters were generated. Conclusion At present, the field of EM-associated infertility is in the development stage, and research hotspots mainly focus on mechanism research and clinical treatment. The follow-up can establish a chronic disease management model to promote the further development in this field.

38 Relationship of serum miR-124-3p and GATA3 expression levels with disease severity and Th1/Th2 balance in children with allergic rhinitis

Objective To investigate the correlation between the expression of serum microRNA-124-3p (miR-124-3p) and GATA binding protein 3 (GATA3) and the severity of the disease and the balance of type Ⅰ helper T cells (Th1)/type Ⅱ helper T cells (Th2) in children with allergic rhinitis (AR). Methods A total of 87 children with AR diagnosed in the Department of Otolaryngology Children’s Health Care of Wuxi Maternal and Child Health Care Hospital (hereinafter referred to as “our hospital”) from June 2020 to April 2022 were selected as the observation objects (AR group), and were divided into mild group (53 cases), moderate and severe group (34 cases) according to the severity of symptoms, and 85 healthy children undergoing physical examination in our hospital during the same period were selected as the control group. Real-time fluorescence quantitative PCR (qRT-PCR) was used to detect the expression level of serum miR-124-3p, and flow cytometry was used to detect the proportion of Th1, Th2 cells, and Th1/Th2. Serum levels of interferon-γ (IFN-γ), interleukin-12 (IL-12), IL-4, IL-5, IL-10 and GATA3 were detected by enzyme-linked immunosorbent assay. The correlation between the expression of miR-124-3p and GATA3 in children with AR was analyzed by Pearson method, while miR-124-3p and GATA3 were associated with Th1/Th2 and cytokines. Results The expression level of miR-124-3p in AR group was lower than that in control group, while the level of GATA3 was higher than that in control group, and the differences were statistically significant (P < 0.05). The expression level of miR-124-3p in the moderate and severe group were lower than that in the mild group, while the level of GATA3 was higher than that in the mild group, and the differences were statistically significant (P<0.05). The proportion of Th1 cells and Th1/Th2 ratio in AR group were lower than that in control group, while the proportion of Th2 cells was higher than that in control group, and the differences were statistically significant (P<0.05). The serum levels of IFN-γ and IL-12 in AR group were lower than those in control group, and the levels of IL-4, IL-5 and IL-10 in AR group were higher than those in control group, and the differences were statistically significant (P<0.05). Correlation analysis showed that the expression level of miR-124-3p in serum of children with AR was negatively correlated with the level of GATA3 (r<0, P<0.05). miR-124-3p was positively correlated with Th1, Th1/Th2, IFN-γ, and IL-12 (r>0, P<0.05), and negatively correlated with Th2, IL-4, IL-5, and IL-10 (r<0, P<0.05). GATA3 was negatively correlated with Th1, Th1/Th2, IFN-γ, and IL-12 (r<0, P<0.05), and positively correlated with Th2, IL-4, IL-5, and IL-10 (r>0, P<0.05). Conclusion Serum miR-124-3p expression level decreased and GATA3 level increased in children with AR, which was related to the severity of the disease and Th1/Th2 imbalance.

43 Research progress on western medicine first-line, traditional Chinese medicine combined with western medicine first-line in the treatment of extensive stage small cell lung cancer

This article summarizes clinical research on western medicine first-line, traditional Chinese medicine combined with western medicine first-line in the treatment of extensive stage small cell lung cancer in recent years. It is found that compared to the single first-line treatment of western medicine, the combination treatment of traditional Chinese medicine, characterized by differentiation of symptoms and treatment, has shown therapeutic advantages such as increasing efficacy, reducing toxicity, prolonging survival, improving traditional Chinese medicine symptoms, and enhancing immunity. It is suggested that traditional Chinese medicine should not only serve as a backup option for the failure of western medicine treatment, but should intervene as early as possible and participate throughout the entire process, forming a complementary traditional Chinese medicine combined treatment model with modern medical models, and achieving maximum benefits.

47 Research progress of traditional Chinese medicine on the comorbidity of psoriasis and diabetes mellitus

Psoriasis belongs to the category of “white crust” in traditional Chinese medicine, and diabetes mellitus is called “consumptive thirst” in traditional Chinese medicine. Psoriasis is more likely to be associated with diabetes mellitus in clinic, which makes the condition complicated. It has a broad development prospect to take traditional Chinese medicine as a starting point to treat the comorbidity. From the perspective of western medicine, this article briefly describes that psoriasis and diabetes mellitus have significant correlation in modern biological basis and risk factors. From the perspective of traditional Chinese medicine, by summarizing the understanding of various medical experts, it can be concluded that dampness evil is at the beginning of the onset, blood stasis runs through the entire process, and later the body’s qi and yin are damaged, “dampness, stasis, and deficiency” are the key causes and pathogenesis of both comorbidities. This paper also expounds the characteristics of traditional Chinese medicine syndrome differentiation in the treatment of psoriasis and diabetes mellitus, in order to better guide clinical practice and give full play to the characteristics of traditional Chinese medicine treatment.

51 Research progress on modern Chinese medicine psychotherapy in the treatment of depression

With the increasing incidence rate of depression year by year, the current mainstream treatment and medication based on western psychological ideas cannot fully meet the needs of depression patients in China. Therefore, traditional Chinese medicine practitioners have kept pace with the times, combined with the characteristics of our current humanistic background, and under the guidance of Chinese medicine psychological theory, the traditional Chinese medicine psychological therapy has been inherited and localized research. By fully absorbing the essence of western natural science, psychiatry, and psychology, modern Chinese medicine psychological therapy has emerged as the times require. Clinically common therapies include low-impedance idea introduction therapy, modified traditional Chinese medicine emotion therapy, traditional Chinese medicine cognitive therapy, and space shifting technology.

55 Research progress of liquid biopsy in the neoadjuvant treatment of rectal cancer

Liquid biopsy is an emerging detection technique with the advantages of being less invasive, highly specific, and avoiding tumor heterogeneity and it used in early screening, efficacy assessment, and recurrence monitoring of tumors. For patients with low to intermediate locally advanced rectal cancer (LARC) receiving neoadjuvant therapy, determining the efficacy of neoadjuvant therapy and monitoring tumor recurrence determines treatment options. Several studies demonstrate that liquid biopsy can predict the effectiveness of neoadjuvant treatment in patients with LARC, monitor tumor recurrence earlier than tumor markers such as carcinoembryonic antigen and imaging, and show the long-term survival prognosis of patients, thus providing a more personalized treatment strategy for patients with LARC receiving neoadjuvant therapy. This article reviews and summarizes the use of liquid biopsy in the neoadjuvant treatment of LARC, focusing on several indicators in liquid biopsy.

59 Research progress on the anticancer effects of berberine by targeting Akt kinase

The anti-tumor effects of berberine (BBR) have been confirmed by a large number of studies, among which Akt kinase mediated signaling pathway is the key pathway of BBR anti-tumor research. Its mechanism of action mainly includes mediating cell cycle arrest, inducing mitochondrial apoptosis and autophagy, preventing epithelial mesenchymal transition and angiogenesis, and improving the sensitivity of chemotherapy drugs. Regulation of cancer cell metabolism and improvement of tumor microenvironment. This article reviews the anticancer effect of BBR by targeting Akt kinase, discusses the anti-tumor mechanism of BBR, and promotes the research progress of BBR in the field of clinical anticancer drugs.

63 Research progress of enhanced recovery after surgery in diabetic mellitus patients

The concept of enhanced recovery after surgery (ERAS) has been proposed for many years and has been widely used in surgery, but the application of ERAS is still in its infancy due to factors such as difficulty in blood glucose control, frequent postoperative complications, and possible delayed gastric emptying in diabetic mellitus patients. After studying the characteristics of perioperative management of diabetic mellitus patients under the concept of ERAS and the recovery of patients after different surgical procedures, it was found that the application of ERAS in diabetic mellitus patients is safe and effective. However, specific measures for perioperative management still lack targeted and systematic standards at this stage, and further research and exploration are needed.

68 Research progress on the role of histone modification in the pathogenesis of systemic lupus erythematosus

The occurrence and development of systemic lupus erythematosus(SLE) are regulated by epigenetic mechanisms, and histone modifications also play an important role in disease progression. SLE patients exhibit changes in histone modification patterns of autoreactive T cells and B cells, including methylation (CREMα, CXCR5, HPK1, EZH2, TNFAIP3, BACH2, etc.), acetylation (BCL-6, BACH2, IL-2/10/17, etc.), phosphorylation, ubiquitination, etc. And these changes may be caused by changes in the level of specific histone modifying enzymes, while the role of histone modifying enzymes and changes in histone modifying levels in SLE is currently in the exploratory stage. This article provides a brief overview of the research progress of histone modification in SLE, and introduces therapeutic drugs targeting epigenetic targets.

72 Evaluation of depression and anxiety in patients with conjunctivochalasis

Objective To investigate the changes of emotional states such as depression and anxiety in patients with conjunctivochalasis. Methods A total of 260 patients diagnosed with conjunctivochalasis were collected from the Department of Ophthalmology, Putuo Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from September 2020 to February 2022. The depression and anxiety status were assessed by patient health questionnaire (PHQ-9) and general anxiety disorder scale (GAD-7), respectively, and the degree of ocular surface discomfort was assessed by ocular surface disease index (OSDI). Schirmer Ⅰ test (SIT), tear film rupture time (BUT), and corneal fluorescein staining (FL) were used to evaluate ocular surface signs. Results There were significant differences in age and proportion of diabetes among different clinical grades (P<0.05). The PHQ-9 and GAD-7 scores of patients with different clinical grades were compared, and the differences were statistically significant (P<0.05). Correlation analysis showed that OSDI score was posi- tively correlated with PHQ-9 and GAD-7 scores (r>0, P<0.05). SIT was negatively correlated with PHQ-9 and GAD-7 scores (r<0, P<0.05). BUT was negatively correlated with PHQ-9 and GAD-7 scores (r<0, P<0.05). FL scores were positively correlated with PHQ-9 and GAD-7 scores (r>0, P<0.05). Logistic regression analysis showed that PHQ-9 and GAD-7 scores were independent risk factors for conjunctival relaxation (OR=1.350, 1.852, P<0.05). There was statistical significance in the proportion of depression and anxiety among different genders (P<0.05). Conclusion The higher the clinical grade, the more serious the degree of depression and anxiety, and the more severe the symptoms of ocular surface discomfort. Women are more likely to have depression and anxiety than men. In the treatment of conjunctivochalasis, we should pay attention to the evaluation of the patient’s psych- ological state, so as to achieve the comprehensive treatment of the disease.

76 Analysis of influencing factors of abnormal bone mass of Cadres of organs directly under Xinjiang Uygur Autonomous Region

Objective To study the abnormal bone mass of cadres in Xinjiang Uygur Autonomous Region who underwent physical examination in the People’s Hospital of Xinjiang Uygur Autonomous Region, and to analyze its risk factors, so as to provide reference for the prevention and treatment of osteoporosis. Methods A total of 1 102 cadres from the district offices who underwent physical examination in the People’s Hospital of Xinjiang Uygur Autonomous Region from January 2020 to December 2021 were selected as the study subjects. The age, sex, and chronic diseases of the subjects were investigated by questionnaire. Physical examination and bone density examination were performed on the subjects. The subjects were divided into three groups according to bone mineral density examination: normal bone mass group (861 subjects), bone loss group (109 subjects), osteoporosis group (132 subjects); logistic regression was used to analyze the risk factors of abnormal bone mass. Results The total prevalence of osteopenia was 9.89%, while the total prevalence of osteoporosis was 11.98%. There were significant differences in gender distribution, age, low density lipoprotein cholesterol (LDL-C), 25- hydroxyvitamin D[25 (OH) D] among the three groups (P<0.05). Logistic regression analysis showed that 25 (OH) D was a protective factor for bone loss (OR [95%CI] =0.920 [0.888-0.953], P<0.05), LDL-C was a risk factor for bone loss (OR [95%CI] =1.431 [1.116- 1.835], P<0.05); age (OR [95%CI] =1.075 [1.053-1.098], P<0.05), LDL-C(OR [95%CI] =1.334 [1.054-1.689], P<0.05) were risk factors for osteoporosis. 25 (OH) D (OR [95%CI] =0.914 [0.883-0.946], P<0.05) and male (OR [95%CI] =0.302 [0.160- 0.570], P<0.05) were protective factors for osteoporosis. Conclusion The prevalence rate of osteoporosis of cadres aged 50 and above in the district offices is high, so pay attention to health examination.

81 Study on sleep and mental characteristics of insomnia patients with sleep maintenance difficulties

Objective To compare the sleep and mental characteristics between sleep maintenance difficulty insomnia and other subtypes of insomnia. Methods From November 2022 to March 2023, patients with insomnia with sleep maintenance difficulties who were admitted to the general Clinic of the Third Affiliated Hospital of Beijing University of Chinese Medicine and the Sleep Disorder Clinic of the Department of Encephalopathy, the Famous Clinic of Beijing First Integrated Traditional and Western Medicine Hospital, the Traditional Chinese Medicine Hall of Beijing University of Chinese Medicine, the Huangjia Medical Circle Hospital of Traditional Chinese Medicine and the social recruitment through online posters were collected (observation group, 141 patients) and other subtypes of insomnia (60 patients in the control group). General information, insomnia severity index (ISI), Pittsburgh sleep quality index (PSQI), and its factor scores, self-rating anxiety scale (SAS), and self-rating depression scale (SDS) were compared between the two groups. Results There were significant differences in age, marital status, educational level, and thyroid disease between the two groups (P<0.05). The sleep quality, time to fall asleep, sleep time, sleep efficiency, and hypnotic drugs in ISI, PSQI total score, and PSQI factor scores were compared between the two groups, and the differences were statistically significant (P<0.05). The results of multiple linear regression showed that sleep maintenance difficulty insomnia was the influencing factor of sleep time, sleep efficiency, sleep disorder, and SDS scores in ISI and PSQI factors (P<0.05). Conclusion Sleep maintenance difficulty insomnia can lead to similar levels of insomnia and depression and anxiety as the sum of other subtypes, and the clinical damage is more serious.

Objective To understand the status quo of recognition of the factors of post competence of preventive medicine students in Shandong Province, and to explore the influencing factors of the consistency between them and experts’ suggestions. Methods From April to June 2021, Shandong Province were divided into three parts according to the region: east, middle, and west. Two schools were selected for each part. Stratified random cluster sampling method was used to select fresh graduates of preventive medicine majors from medical colleges and universities to fill in the questionnaire. Multivariate logistic regression was used to analyze the influencing factors of the inconsistency between the recognition of relevant factors of post competence of preventive medicine students and expert suggestions. Results A total of 1 549 questionnaires were collected, among which 1 488 were valid, with an effective recovery rate of 96.06%. The total score of relevant factor identification index of post competence of preventive medicine major students was (3.46±0.68) points, the score of professional accomplishment was (3.62±0.71) points, and the score of professional knowledge and skills was (3.30±0.86) points. There were significant differences in gender and proportion of student cadres between the two groups (P<0.05). The results of multivariate regression analysis showed that gender (OR=0.419) and serving as student cadre (OR=0.627) were the influencing factors for the inconsistency between the identification of relevant elements of post competency and expert recommendations of preventive medicine students (P<0.05). Conclusion Emphasis should be placed on the cultivation of various comprehensive qualities and abilities of preventive medicine students, and the construction of public health pers- onnel should be strengthened.

Objective To investigate the effects of Dulaglutide on glycemic variability and microinflammatory state in patients with type 2 diabetes mellitus poorly controlled by Metformin. Methods A total of 118 patients with type 2 diabetes mellitus who were poorly controlled by metformin alone in Rugao People’s Hospital, Nantong University from January 2019 to January 2022 were selected as the research objects. They were divided into control group and study group by random number table method, with 59 cases in each group. The control group was treated with Metformin combined with Insulin Aspart 30, and the study group was treated with Dulaglutide on the basis of the control group. Blood glucose biochemical indexes, inflammatory indexes, islet function indexes, and drug safety were evaluated after three months of treatment in both groups. Results After treatment, the fasting blood glucose, 2-hour postprandial blood glucose, and glycosylated hemoglobin of the two groups were lower than those before treatment, and the proportion of time in range (TIR) of glucose target was higher than that before treatment, and the TIR of the study group was higher than that of the control group (P<0.05). After treatment, the levels of interleukin-6 and high-sensitivity C-reactive protein in two groups were lower than those before treatment, and those in the study group were lower than those in the control group (P<0.05). After treatment, homeostasis model assessment index of islet β cell (HOMA-β) in both groups were higher than those before treatment, homeostasis model assessment index of insulin resistance (HOMA-IR) were lower than those before treatment, HOMA-β in study group was higher than that in control group, HOMA-IR was lower than that in control group (P<0.05). The incidence of nausea and vomiting in the study group was higher than that in the control group (P<0.05). Conclusion The application of Dulaglutide in the treatment of patients with type 2 diabetes mellitus poorly controlled by Metformin can help to control blood glucose fluctuation and inhibit the expression of inflammatory factors, but it may increase nausea and vomiting.

93 Systematic evaluation of the treatment of knee osteoarthritis by holistic bone setting and therapeutic manipulation for injured soft tissue

Objective To systematically evaluate the efficacy and safety of holistic bone setting and therapeutic manipulation for injured soft tissue in the treatment of knee osteoarthritis (KOA) by meta-analysis. Methods PubMed, Web of Science, CNKI, Wanfang Data, and VIP were searched for the literatures evaluating the holistic bone setting and therapeutic manipulation for injured soft tissue in the treatment of KOA published from the inception to December 2022. The literatures’ information was extracted, and RevMan 5.4 was used for meta-analysis. Results A total of 12 literatures involving 922 KOA patients were included, including 464 in the treatment group and 458 in the control group. The intervention measures of the treatment group were holistic bone setting and therapeutic manipulation for injured soft tissue, and the intervention measures of the control group were local knee joint manipulation. Meta-analysis showed that the total effective rate of the treatment group was higher than that of the control group (RD=0.15, 95%CI: 0.10 to 0.20, P<0.000 01). Visual analogue scale score (MD=-0.80, 95%CI: -0.97 to -0.63, P<0.000 01), Western Ontario and McMaster University osteoarthritis pain score (MD=-1.19, 95%CI: -1.70 to -0.68, P<0.000 01), Western Ontario and McMaster University osteoarthritis stiffness score (MD=-0.61, 95%CI: -0.91 to -0.31, P<0.0001), Western Ontario and McMaster University osteoarthritis dysfunction score (MD=-6.27, 95%CI: -7.55 to -4.98, P<0.000 01) in the treatment group were lower than those in the control group. There was no significant difference in the total incidence of adverse reactions between the two groups (RD=0, 95%CI: -0.02 to 0.03, P=0.83). Conclusion Holistic bone-setting and therapeutic manipulation for injured soft tissue for KOA is superior to local manipulation for knee joint in terms of effectiveness and symptom function improvement. Limited by the number and quality of included studies, more high-quality, large-sample studies are needed to verify this.

100 Clinical effect of Bushen Tongbi Decoction in the treatment of knee osteoarthritis with liver and kidney deficiency

Objective To explore the clinical effect of Bushen Tongbi Decoction in the treatment of knee osteoarthritis with liver and kidney deficiency. Methods A total of 90 knee osteoarthritis patients with liver and kidney deficiency treated in Shijiazhuang People’s Hospital from March 2019 to April 2020 were selected, and they were divided into control group and experimental group by random number table method, with 45 cases in each group. The control group was treated by Diclofenac Sodium, and the experimental group was treated by Bushen Tongbi Decoction on the basis of control group, for 30 days. The visual analogue scale(VAS) scores for knee pain,Western Ontario and McMaster University Osteoarthritis Index (WOMAC) scores, clinical effect and levels of serum interleukin-6 (IL-6), tumor necrosis factor-α(TNF-α) were compared between the two groups. Results After treatment,VAS and WOMAC scores in the two groups were lower than before treatment, and those in the experimental group were lower than those of the control group, the differences were statistically significant (P<0.05). The clinical effect of the experimental group was better than that of the control group, the difference was statistically significant (P<0.05). After treatment, the levels of serum IL-6 and TNF-α in two groups were lower than before treatment, and those of the experimental group were lower than those of the control group, the differences were statistically significant(P<0.05). Conclusion Bushen Tongbi Decoction can effectively relieve knee pain, improve dysfunction, and reduce the inflammation levels of knee osteoarthritis with liver and kidney deficiency, the effect is remarkable.

104 Study on the correlation between perimenopause and metabolic syndrome

Objective To explore the correlation between perimenopause and metabolic syndrome (MS). Methods Using the method of cross-sectional study, the data of physical examination of women aged 40-60 years in Beijing Xiaotangshan Hospital from 2014 to 2017 were selected. The clinical data of subjects were collected and the influencing factors of MS in perimenopausal women were analyzed by logistic regression model. Results There were statistically significant differences in the prevalence of MS among different ages, educational background, working pressure, dietary taste, eating speed, and menopausal conditions (P<0.05). Analysis of risk factors for MS in women not included in menopause showed that the prevalence of MS in women aged >46-53 was 1.784 times that of women aged 40-46, and the prevalence of MS in women aged >53-60 was 2.631 times that of women aged 40-46; the prevalence of MS in women with the highest education of junior college or undergraduate was 0.539 times that of junior high school or below, and the prevalence of MS in women with the highest education of graduate school or above was 0.262 times that of junior high school or below; women who ate breakfast three to five times a week were 0.322 times more likely to have MS than women who ate breakfast three times a week; women who ate fast were 2.701 times more likely to have MS than those who ate slowly. Analysis of risk factors for MS in women without age factors showed that the prevalence of MS in women with the highest education of graduate school or above was 0.141 times that of junior high school or below; the prevalence of MS in women with moderate eating speed was 2.483 times that of eating slowly, and that in women with fast eating speed was 3.572 times that of eating slowly; the incidence of MS in postmenopausal women was 1.808 times that of pre-menopausal women. Conclusion Perimenopause is related to the occurrence of MS. Perimenopausal women should prevent and treat MS in advance to improve the quality of life before and after menopause.

108 Expression and clinical significance of serum miR-29a and miR-143-3p in patients with polycystic ovary syndrome

Objective To analyze the relationship between serum microRNA (miRNA)-29a and miR-143-3p expression and obesity, glucolipid metabolism, and insulin resistance (IR) in patients with polycystic ovary syndrome (PCOS). Methods A total of 86 patients with PCOS admitted to Zhongshan Hospital Affiliated to Xiamen University (hereinafter referred to as “our hospital”) from June 2019 to June 2022 were selected as the PCOS group, and 49 healthy volunteers with physical examination in our hospital during the same period were selected as the control group. PCOS patients were divided into the obese group (36 cases) and the non-obese group(50 cases) according to body mass index (BMI), and they were divided into the IR group (61 cases) and the non-IR group (25 cases) according to homeostasis model assessment (HOMA)-IR. Serum miR-29a, miR-143-3p expression and obesity indicators (BMI, waist-to-hip ratio [WHR]), glucolipid metabolism indicators (fasting blood glucose [FBG], fasting insulin [FINS], total cholesterol [TC], triacylglycerol [TG], high-density lipoprotein cholesterol [HDL-C], and low-density lipoprotein cholesterol [LDL-C]) and HOMA-IR of all groups were compared; the correlation between serum miR-29a and miR-143-3p expression and obesity indicators, glucolipid metabolism indicators, and HOMA-IR in PCOS patients were analyzed. Results BMI, WHR, FBG, FINS, HOMA-IR, TC, TG, LDL-C, miR-29a, and miR-143-3p in PCOS group were higher than those in control group, and HDL-C was lower than that in the control group (P<0.05). Serum miR-29a and miR-143-3p expression in the obese group were higher than those in the non-obese group (P<0.05). Serum miR-29a and miR-143-3p expression in the IR group were higher than those in the non-IR group (P<0.05). Serum miR-29a and miR-143-3p expression in PCOS patients were positively correlated with BMI, WHR, FBG, FINS, HOMA-IR, TC, TG, and LDL-C (r/rs>0, P<0.001) and HDL-C was negatively correlated (r<0, P<0.001). Conclusion High expression of serum miR-29a and miR-143-3p in PCOS patients is closely associated with obesity, glucose and lipid metabolism, and IR.

Objective To investigate the relationship between MTHFR C677T gene mutation, and the levels of serum homocysteine (Hcy), folic acid, and vitamin B12 with spontaneous abortion in Uygur pregnant women in Kashgar area. Methods From January 2020 to June 2021, 75 spontaneous abortion patients in the Reproductive Medicine Center of the First People’s Hospital of Kashi, Xinjiang Uygur Autonomous Region (hereinafter referred to as “our hospital”) were selected as the observation group, and 75 healthy pregnant women who underwent physical examination in our hospital during the same period were selected as the control group. The polymorphism of MTHFR C677T locus gene and the levels of serum Hcy, folic acid, and vitamin B12 between two groups were detected; the levels of serum Hcy, folic acid, and vitamin B12 levels among different MTHFR genotypes were compared; the influencing factors of spontaneous abortion among Uyghur pregnant women in Kashgar area were analyzed. Results The proportion of induced abortion history, infection in early pregnancy, and the number of induced abortion in the observation group were higher than those in the control group (P<0.05). There were significant differences in genotype and allele distribution between the two groups (P<0.05). The level of serum Hcy in the observation group was higher than that in the control group, and the levels of folic acid and vitamin B12 were lower than those in the control group (P<0.05). The level of serum Hcy of TT genotype was higher than that of CT and CC genotype, and the levels of folate and vitamin B12 were lower than those of CT and CC genotype (P<0.05); there were no significant differences in the levels of serum Hcy, folic acid, and vitamin B12 between CT and CC genotype patients (P>0.05). TT genotype and high Hcy level were risk factors for spontaneous abortion in Uyghur pregnant women in Kashgar area (OR=2.044, 2.010, P<0.05), and high folic acid and vitamin B12 levels were protective factors for spontaneous abortion in Uyghur pregnant women in Kashgar area (OR=0.587, 0.614, P<0.05). Conclusion MTHFR C677T genotype, the levels of serum Hcy, folic acid, and vitamin B12 are associated with spontaneous abortion in Uyghur pregnant women in Kashgar area.

117 Effect of longsha open and close pivot needle method combined with Letrozole in the treatment of infertility with polycystic ovarian syndrome

Objective To explore the therapeutic effect of longsha open and close pivot needle method combined with Letrozole on infertility with polycystic ovary syndrome (PCOS). Methods A total of 84 infertility patients with PCOS admitted to Wuxi Hospital of Traditional Chinese Medicine, Jiangsu Province from June 2021 to October 2022 were selected, and they were divided into control group (received Letrozole treatment) and observation group (received longsha open and close pivot needle method combined with Letrozole treatment), with 42 cases in each group. The clinical efficacy of two groups were compared; ovulation rate and pregnancy rate of two groups were compared; ovarian volume, endometrial thickness, serum sex hormones (testosterone [T], estradiol [E2], luteinizing hormone [LH], follicle-stimulating hormone [FSH]) of two groups before and after treatment were compared; and the occurrence of adverse reactions during treatment was observed. Results The clinical efficacy of observation group was better than that of control group (P<0.05). There was no difference in ovulation rate between two groups (P>0.05); the pregnancy rate of observation group was higher than that of control group (P<0.05). After treatment, the ovarian volume of two groups was lower than that before treatment, and the endometrial thickness was higher than that before treatment (P<0.05); the ovarian volume of observation group was lower than that of control group, and the endometrial thickness was higher than that of control group (P<0.05). After treatment, the serum T, LH, and FSH levels in two groups were decreased than those before treatment, while E2 levels were increased than those before treatment (P<0.05); and the serum T, LH and FSH levels in observation group were lower than those of control group, while E2 levels were higher than those of control group (P<0.05). There was no difference in the total incidence of adverse drug reactions between two groups (P>0.05). Conclusion Longsha open and close pivot needle method combined with Letrozole in the treatment of infertility with PCOS can improve clinical efficacy, increase pregnancy rate, improve endometrial receptivity, regulate sex hormone levels, and is safe and reliable.

121 Clinical application of Tongqiao Biyan Granules in allergic rhinitis patients with syndrome of wind-heat invading lung

Objective To observe the clinical application of Tongqiao Biyan Granules in allergic rhinitis patients with syndrome of wind-heat invading lung. Methods A total of 102 allergic rhinitis patients with syndrome of wind-heat invading lung admitted to the North District, Suzhou Municipal Hospital, Jiangsu Province from August 2019 to August 2022 were selected and divided into combined group (51 cases) and routine group (51 cases) by random number table method. The routine group was treated with Levocetirizine, and the combined group was treated with Levocetirizine combined with Tongqiao Biyan Granules. Both groups were treated for three weeks. The clinical efficacy, and symptom score, sign score, olfactory recognition threshold before and after treatment were compared between the two groups, and the occurrence of adverse reactions were recorded. Results The clinical effect of the combined group was better than that of the routine group, and the difference was statistically significant (P<0.05). After treatment, the symptoms, signs scores, and olfactory recognition threshold of the two groups were lower than those before treatment, and those of combined group were lower than those of control group, and the differences were statistically significant (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05). Conclusion Tongqiao Biyan Granules combined with Levocetirizine can improve the clinical symptoms of allergic rhinitis patients with syndrome of wind-heat invading lung, and can reduce the olfaction recognition threshold of patients, and is safe, worthy of promotion.

Methods Sixty moderate and severe chronic periodontitis patients with syndrome of yang deficiency of spleen and kidney admitted to Wuhan Hospital of Traditional Chinese Medicine from April 2021 to May 2022 were selected as the study objects, and they were divided into control group (periodontal basic treatment) and observation group (periodontal basic treatment combined with Fupi Yishen Decoction) according to random number table method, with 30 cases in each group. The effects of the two groups were compared, and the differences of traditional Chinese medicine syndrome scores, periodontal indicators, and inflammatory factor indicators between the two groups were compared before treatment and one month after treatment, and the occurrence of adverse reactions during treatment between the two groups were recorded. Results The clinical efficacy of the observation group was better than that of the control group (P<0.05). After one month of treatment, the scores of tooth loosening and displacement, gingival swelling, bleeding, and mastication weakness in two groups were lower than those before treatment, and those of observation group was lower than those of control group (P<0.05). After one month of treatment, periodontal attachment level, plaque index, and periodontal pocket depth in two groups were lower than those before treatment, and those of observation group were lower than those of control group (P<0.05). After one month of treatment, the gingival crevicular fluid tumor necrosis factor-α, interleukin-1β, interleukin-6, and serum C-reactive protein in two groups were lower than those before treatment, and those of observation group were lower than those of control group (P<0.05). No obvious adverse reactions were observed in both groups during treatment. Conclusion Fupi Yishen Decoction adjuvant treatment of moderate and severe chronic periodontitis with syndrome of yang deficiency of spleen and kidney, can promote the improvement of symptoms, and periodontal related indicators, reduce the level of inflammatory factors, the effect is significant.

130 Effect of Yunnan Hongyao Capsules in the treatment of patients with retinal vein obstruction of qi stagnation and blood stasis and Qi-Yin deficiency syndrome

Objective To explore the effect of Yunnan Hongyao Capsules in treating patients with retinal vein obstruction of Qi stagnation and blood stasis and Qi-Yin deficiency syndrome. Methods Fifty hospitalized patients with retinal vein occlusion and fundus hemorrhage admitted to the Department of Ophthalmology, Yunnan Provincial Hospital of Traditional Chinese Medicine from June 2019 to May 2020 were divided into control group and experimental group according to random number table method, with 25 cases in each group. The control group and the experimental group were treated with Calcium Hydroxybenzene Sulfonate and Yunnan Hongyao Capsules respectively. The visual acuity level, fundus condition, macular edema, overall treatment effect, and coagulation function indexes were compared between the two groups. Results After treatment, the visual acuity level of the two groups were higher than those before treatment, while that of the experimental group was higher than that of the control group, and the differences were statistically significant (P<0.05). The improvement of fundus in experimental group was better than that in control group, and the difference was statistically significant (P<0.05). After treatment, macular edema in the two groups were lower than before treatment, while that of the experimental group was lower than that of the control group, and the differences were statistically significant (P<0.05). The overall treatment effect of the experimental group was better than that of the control group, and the difference was statistically significant (P<0.05). After treatment, the thrombin time, activated partial thrombin time, and prothrombin time of the two groups were shorter than before treatment, and the fibrinogen time was lower than before treatment, while the thrombin time, activated partial thrombin time, and prothrombin time of the experimental group were longer than those of the control group, and the fibrinogen time of the experimental group was higher than that of the control group, and the differences were statistically significant (P<0.05). Conclusion In the treatment of fundus hemorrhage with retinal vein obstruction, Yunnan Hongyao Capsules combined with Calcium Hydroxybenzene Sulfonate can effectively improve the visual acuity level, fundus condition, and coagulation function index, thereby reducing the thickness of macular fovea and improving the therapeutic effect.

Tumor-related depression has the characteristics of high incidence and low treatment rate, which is a disadvantageous factor for the prognosis of tumor patients. Exploring its reasonable treatment plan can improve the quality of life and long-term prognosis of patients. The integral view of body, qi, and spirit, which is rooted in ancient Chinese philosophy, is an important embodiment of Chinese medical image thinking and plays a programmatic guiding role in the treatment of diseases. Professor Zhang Peitong concludes that the core pathogenesis of tumor-related depression is liver qi stagnation,with zang-fu viscera deficiency, phlegm, blood stasis, and cancer toxin cementation, and the main location of the disease is the liver, which is closely related to the heart and spleen. He emphasizes distinctive pathological changes of tumor-related depression in the three dimensions of body, qi, and spirit, and believes that the loss of body and spirit is an important pathogenesis of tumor-related depression, and the disorder of qi is the intermediate link in the comorbidity of body and spirit diseases. Attacking the shape of the tumor is the cornerstone of the overall treatment plan. The filling and regulating of qi should be carried out throughout the treatment, and the purpose of regulating the function of the zang-fu viscera should be achieved in time. Therefore, the treatment principles of removing form as the basis, always regulating qi, and regulating the spirit in time are proposed to restore the human body to a peaceful state of shape health, qi regulation, and spirit peace, trying to provide a better idea for the comprehensive treatment of tumor-related depression.

Objective To explore the effect of Tiaowei Chengqi Prescription umbilical therapy combined with deep breathing exercises and abdominal massage on treating of prevention constipation of hip fractures in middle-aged and elderly people during the perioperative. Methods A total of 63 middle-aged and elderly patients with hip fracture were selected from Wangjing Hospital of China Academy of Chinese Medical Sciences from April 2021 to June 2022, and they were divided into experimental group 32 cases, and control group 31 cases by random number table method. The two groups were given conventional treatment, and the experimental group was given Tiaowei Chengqi Prescription umbilical therapy combined with deep breathing exercises and abdominal massage on the basis of conventional treatment. The clinical efficacy and impact on the quality of life of patients in two groups were observed. Results The clinical efficacy of the experimental group was better than that of the control group, the difference was statistically significant (P<0.05). After treatment, the Wexner constipation and Bristol fecal trait scores of the two groups were lower than those before treatment, and the experimental group was lower than the control group (P<0.05). The defecation cycle of the control group was longer than before treatment, and the defecation cycle of the experimental group was shorter than that of the control group, and the differences were statistically significant (P<0.05). After treatment, the positive coping score of the two groups was higher than those before treatment, the negative coping scores of the two group were lower than those before treatment, and the positive coping score of the experimental group was higher than that of the control group, and the negative coping score was lower than that of the control group, the differences were statistically significant (P<0.05). One month after discharge, the physical and mental scores of the two groups were higher than those before treatment, and the group was higher than the control group, the differences were statistically significant (P<0.05). Conclusion The Tiaowei Chengqi Prescription umbilical therapy combined with deep breathing exercises and abdominal massage has a good effect on the treatment of perioperative hip fracture bedridden constipation in middle-aged and elderly patients, and can improve the quality of life of patients.

Malignancy-associated gastroparesis is an important factor affecting the quality of life of patients with malignant tumors and is one of the common complications after surgery and radiotherapy. Western medicine treatment for this disease is limited and has unsatisfactory efficacy. Therefore, exploring treatment strategies in traditional Chinese medicine for this disease is of important clinical significance. Professor Cao Yang believes that the basic pathogenesis of this disease is spleen-yang and stomach-yin deficiency, and excessive cold and heat in the body affected by surgery and radiotherapy, accompanied by qi stagnation and phlegm dampness, and should be treated based on cold and heat syndrome differentiation. When medication, warm traditional Chinese medicine or heat-clearing drugs are chosen according to symptoms while promoting the circulation of qi and resolving dampness. Traditional Chinese medicine ointment rubbing is used as the main treatment method during the fasting period of patients, coordinated with other external therapies as appropriate. The comprehensive treatment plan with oral administration of concentrated traditional Chinese medicine decoction is added after oral feeding, which achieves high efficacy in clinical application, broadening the thinking for traditional Chinese medicine treatment of malignancy-associated gastroparesis. At the end of our article, a proven case is attached to verify its effectiveness.

Characteristic diagnosis and treatment techniques of folk traditional Chinese medicine are the source of emergence and development of traditional Chinese medicine, which play an important role in protecting public health. However, characteristic diagnosis and treatment techniques of folk traditional Chinese medicine are good at practice but short in theory. The author analyzed the principles of four manipulative techniques of folk traditional Chinese medicine, including Zhu’s manipulation of tendons and bonesetting, Li’s iron wrist straightening method, Li’s spinal resetting technique, and Zhang’s spinal plastic reduction technique, and found that the holistic concept is reflected, the importance of spine and du meridian is valued. They all achieve the role of regulating the whole body by correcting the spine-unblocking the du meridian-regulating yang qi. This paper discusses and analyzes the theory of characteristic diagnosis and treatment techniques of folk traditional Chinese medicine, expecting to contribute to its inheritance, innovation and application, so as to improve service ability of traditional Chinese medicine.

151 Clinical effect of floating needle combined with dialectical traction on cervical spondylotic radiculopathy

Objective To explore the clinical effect of floating needle combined with dialectical traction on cervical spondylotic radiculopathy. Methods A total of 70 patients with cervical spondylotic radiculopathy admitted to First Affiliated Hospital, Heilongjiang University of Chinese Medicine from May 2020 to December 2022 were selected as the study objects, and they were divided into the observation group and the control group according to random number table method, with 35 cases in each group. The observation group was treated with floating needle combined with dialectical traction, while the control group was treated with conventional acupuncture combined with dialectical traction. The treatment course was two weeks. The cervical curvature of the two groups was compared (cervical curvature Bordens method, cervical curvature index (CCI) method, and cervical vertebra centroid line (CCL) method measurement). The levels of interleukin (IL)-6, IL-8, tumor necrosis factor-α (TNF-α) inflammatory response factors and hemorheological factors such as whole blood viscosity low cut, whole blood viscosity high cut, plasma viscosity, and fibrinogen were detected. Results After two weeks of treatment, the clinical efficacy of the observation group was better than that of the control group, the difference was statistically significant (P<0.05). After two weeks of treatment, the physiological curvature of the cervical spine between the two groups was lower than before treatment, and the measurement result of CCI method in the observation group was lower than that of the control group, and the differences were statistically significant (P<0.05). There were no significant differences in the measurement results of cervical curvature between the two groups by Bordens method and CCL method (P>0.05). After two weeks of treatment, the expression level of inflammatory factors in two groups was lower than that before treatment, and the difference was statistically significant (P<0.05). After two weeks of treatment, there was no statistically significant difference in the expression level of inflammatory factors between two groups (P>0.05). After two weeks of treatment, the levels of hemorheological related factors in the two groups were lower than before treatment, and the whole blood viscosity, plasma viscosity, and fibrinogen in the observation group were lower than those in the control group, the differences were statistically significant (P<0.05). There was no significant difference in whole blood viscosity between the two groups (P>0.05). Conclusion Floating acupuncture combined with dialectical traction can obviously alleviate the symptoms of cervical radiculopathy patients, restore the physiological curvature of cervical spine, reduce the serum inflammatory factors and hemorheological related indexes, which is worth popularizing.

Objective To explore the curative effect of traditional Chinese medicine Kechuan Mixture on lung function, immune function, and serum inflammatory factors in children with phlegm heat regulated lung syndrome and cough. Methods A total of 102 children with phlegm heat regulated lung syndrome and cough were selected from the Department of Pediatrics, Wujin Hospital of Traditional Chinese Medicine from October 2019 and October 2022, they were divided into control group and study group by random number table method, with 51 cases in each group. The control group was treated with western medicine, and the study group was treated with traditional Chinese medicine Kechuan Mixture on the basis of control group. The efficacy, lung function, immune function and inflammation level before and after treatment were compared between the two groups. Results The efficacy of the study group was better than that of the control group (P<0.05). After treatment, the forced expiratory volume in the one second/forced vital channel and peak expiratory flow of the two groups were higher than before treatment, and the study group was higher than the control group (P<0.05). After treatment, CD3+ and CD4+/CD8+ in both groups were higher than before treatment, and the study group was higher than the control group (P<0.05). After treatment, interleukin-8 and procalcitonin in two groups were lower than before treatment, and the study group was lower than the control group (P<0.05). Conclusion The traditional Chinese medicine Kechuan Mixture can enhance the therapeutic effect of children with pneumonia, asthma, cough, phlegm heat accumulation in the lungs, and improve lung function, immunity, and inflammation levels.

160 Effects of acupuncture and moxibustion on enteral nutrition aspiration, gas- trointestinal function, and inflammatory indexes in critically ill patients

Objective To investigate the effects of acupuncture and moxibustion on enteral nutrition (EN) aspiration, gastrointestinal function, and inflammatory indexes in critically ill patients. Methods A total of 106 critically ill patients from the Department of Critical Care Medicine, Wenzhou Central Hospital of Zhejiang Province from January 2021 to December 2022 were selected and they were divided into acupuncture and moxibustion group and control group according to random number table method, with 53 cases in each group. The control group was given EN support within 24 to 48 h after admission. If there was feeding intolerance, Metoclopramide could be given intramural injection, 10 mg/time, twice a day, for 24 to 48 h. The acupuncture and moxibustion group was treated on the basis of the control group, and moxibustion was selected at zúsānlǐ and zhōngwǎn, hanging moxibustion, twice a day, 15 min/times; acupuncture was selected at two sides of zúsānlǐ, sānyīnjiāo, nèiguān, zhōngwǎn, shàngjùxū, xiàjùxū, tiānshū, and qìhǎi, warming needle moxibustion was selected at zúsānlǐ, or zhōngwǎn. Puncture the acupoint directly and leave the needle for 30 min after arrival of qi, once a day. Both groups were treated for one week. The acute gastrointestinal injury (AGI) grade, inflammatory indexes (interleukin [IL] -6, IL-1β, tumor necrosis factor-α [TNF-α]), and nutritional indexes (albumin [Alb], prealbumin [PA], hemoglobin [Hb]) of the two groups were compared before and after intervention. EN complications and Metoclopramide dosage were analyzed. Results After one week of intervention, AGI grading in both groups was lower than that before intervention, and that in acupuncture and moxibustion group was lower than that in control group, the differences were statistically significant (P<0.05). After one week of intervention, serum IL-6, IL-1β, and TNF-α levels in two groups were lower than those before intervention, and those in the acupuncture group were lower than those in the control group, the differences were statistically significant (P<0.05). After one week of intervention, serum Alb, PA, and Hb levels in two groups were higher than those before intervention, and those in the acupuncture group was higher than those in the control group, the differences were statistically significant (P<0.05). The incidences of aspiration, diarrhea, and abdominal distension in acupuncture and moxibustion group were lower than that in control group, and the differences were statistically significant (P<0.05). The dosage of Metoclopramide in acupuncture and moxibustion group was lower than that in control group, and the difference was highly statistically significant (P<0.01). Conclusion Acupuncture and moxibustion for EN support in critically ill patients can significantly improve gastrointestinal function and nutritional status, reduce inflammatory response, and reduce complications such as aspiration.

164 Clinical effect of Tongtiao Granules on unstable angina pectoris of phlegm-stasis interjunction type

Objective To explore the clinical effect of Tongtiao Granules on unstable angina pectoris of phlegm-stasis interjunction type. Methods A total of 120 patients with unstable angina pectoris treated in Qinhuangdao Hospital of Traditional Chinese Medicine of Hebei Province from April 2019 to January 2022 were selected as the study objects. They were divided into two groups by random number table method, with 60 cases in each group. The control group was treated with conventional western medicine, and the observation group was treated with Tongtiao Granules on the basis of control group, both groups were treated continuously for one course. The clinical effect of two groups was compared; traditional Chinese medicine syndrome scores, 24 h holter electrocardiogram and cardiac remodeling indexes, and Seattle angina questionnaire (SAQ) score were compared between two groups before and after treatment; the occurrence of adverse reactions during treatment was observed. Results The clinical effect of the observation group was better than that of the control group (P<0.05). After treatment, the scores of chest tightness, chest pain, palpitation, shortness of breath, sweating, sticky mouth and body weight, and waist and knee soreness in two groups were lower than those before treatment, and observation group was lower than control group (P<0.05). After treatment, the number of ST segment depression and the maximum ST segment depression in both groups were lower than those before treatment, and observation group was lower than control group (P<0.05); b value and k value of left anterior wall in both groups were higher than those before treatment, and observation group was higher than control group (P<0.05). After treatment, the score of anginal stability, physical limitation, disease awarenes, anginal frequency, and treatment satisfaction, and total score of SAQ in two groups were higher than those before treatment, and observation group was higher than control group (P<0.05). There was no significant difference in the total incidence of adverse reactions between two groups (P>0.05). Conclusion Tongtiao Granules can effectively improve the clinical symptoms and signs of unstable angina pectoris of coronary heart disease, electrocardiogram findings and myocardial revascularization, improve the quality of life, and do not increase adverse reactions during treatment, with high safety.

168 Application effect of early warning model based on BISAP-HDL combined with cluster nursing in patients with acute pancreatitis

Objective To explore the application effect of early warning model based on bedside index for severity in acute pancreatitis-high density lipoprotein (BISAP-HDL) combined with cluster nursing in patients with acute pancreatitis(AP). Methods Eighty AP patients treated in the 904th Hospital of Joint Support Force of Chinese People’s Liberation Army from February 2021 to February 2022 were prospectively selected as the study objects, they were divided into control group (n=42) and observation group (n=38) by random number table method. The control group was given cluster nursing, and the observation group was given early warning model nursing based on BISAP-HDL combined wite cluster. Acute physiological and chronic health scores Ⅱ(APACHEⅡ), BISAP-HDL scores, length of hospital stay, abdominal pain relief time, amylase recovery time, immune function and inflammatory factor levels before and after treatment were compared between the two groups 24 h before treatment and one week after treatment, and the mortality rate at 28 days after admission was compared between the two groups. Results One week after treatment, the scores of APACHEⅡ and BISAP-HDL in both groups were lower than those before 24 h treatment, and the observation group was lower than the control group (P<0.05). The hospitalization time, abdominal pain relief time and hematuric amylase recovery time in observation group were shorter than those in control group (P<0.05). After treatment, CD4+ in both groups was higher than those before treatment, and the observation group was higher than that of the control group (P<0.05). CD8+, immunoglobulin G, interleukin-6, tumor necrosis factor-α, and C-reactive protein in two groups were lower than those before treatment, and observation group was lower than control group (P<0.05). The fatality rate at 28 days after admission in the observation group and the control group was 7.89% (3/38) and 16.67% (7/42), respectively, with no significant difference (P>0.05). Conclusion Early warning model based on BISAP-HDL combined with cluster nursing is beneficial to the recovery of AP patients, improving their immune function and inflammatory factor levels.

Objective To explore the effect of diversified positive emotional arousal nursing on stress response and anesthesia recovery of patients undergoing prostate cancer surgery under general anesthesia. Methods A total of 120 patients with prostate cancer who were treated in the Department of Anesthesia Operating Room, Jiangsu Province Hospital from July 2020 to July 2022 were selected. They were divided into control group and intervention group by random number table method, with 60 cases in each group. The control group received routine anesthesia awakening nursing, while the intervention group received diversified positive emotional arousal nursing on basis of the control group. The Herth hope index score, stress response level, and anesthesia recovery were compared between the two groups before and after intervention. Results After intervention, the scores of positive action, attitude, and intimacy with others in both groups were higher than those before intervention, and the intervention group was higher than the control group, and the differences were statistically significant (P<0.05). Interleukin-6 levels in both groups were lower than before intervention, and the intervention group was lower than the control group, the levels of cortisol and norepinephrine in both groups were higher than those before intervention, and those of the intervention group was higher than the control group, and the differences were statistically significant (P<0.05). The agitation score of both groups was higher than before intervention, and the intervention group was higher than those of the control group; the eye opening time and respiratory recovery time of both groups were shorter than those of the control group, and the differences were statistically significant (P<0.05). Conclusion Diversified positive emotional arousal nursing can improve patients’ hope level for disease treatment, improve the stress response of patients after anesthesia, reduce the agitation after anesthesia, and promote the rapid recovery of anesthesia and breathing.

The control group was given routine nursing, while intervention group was given portable visual nasointestinal tube indwelling system combined with cluster nursing. The catheterization time and success rate of one-time catheterization, nutritional indexes before and after intervention, incidence of unplanned extubation (UEX) and adverse events during nursing, and nursing satisfaction after intervention were compared between the two groups. Results The catheterization time in intervention group was shorter than that in control group, and success rate of one-time catheterization was higher than that in control group (P<0.05). After intervention, the nutritional indexes of both groups were higher than those before intervention, and the intervention group was higher than the control group, and the differences were statistically significant (P<0.05). The incidence of UEX and adverse events in intervention group was lower than that in control group (P<0.05). After intervention, nursing satisfaction in intervention group was better than that in control group (P<0.05). Conclusion The application of portable visual nasointestinal tube indwelling system combined with cluster nursing can shorten catheterization time, increase success rate of one-time catheterization, improve nutritional status, reduce the incidence of UEX and adverse events, and improve nursing satisfaction in severe patients in ICU.

181 Application of “internet + nursing service” in home nursing of cerebral infarction patients

Objective To explore the application of “internet + nursing service” in home nursing of cerebral infarction patients. Methods A total of 94 patients with cerebral infarction admitted to the Sixth People’s Hospital of Nantong from March 2020 to May 2022 were randomly selected by simple sampling method, and they were divided into control group and observation group by random number table method, with 47 cases in each group. The control group implemented routine home nursing, and the observation group implemented “internet + nursing service” mode. The nursing quality, neurological function, and quality of life before and after nursing were observed and compared between the two groups. Results After nursing, the total scores of continuing nursing scale in both groups were higher than those before nursing, and the observation group was higher than control group, (P<0.05). After nursing, the total score of the National Institutes of Health Stroke Scale in both groups was lower than that before intervention, and the observation group was lower than control group (P<0.05). After nursing, the total score of quality of life measurement scale in two groups was higher than that before nursing, and the observation group was higher than control group (P<0.05). Conclusion Carrying out “internet + nursing service” mode during the period of cerebral infarction patients at home can effectively improve the quality of home nursing service and improve the prognosis.

185 Burkholderia pseudomallei pneumonia with sepsis and necrotizing lymphadenitis: a case report and literature review

Melioidosis is a disease caused by the aerobic Gram-negative bacillus Burkholderia pseudomallei, it has a variety of clinical manifestations, of which Burkholderia pseudomallei is the most serious. A case of recurrent fever, cough, and expectoration was presented in the First Affiliated Hospital of Guangdong Medical University. The patient had repeated symptoms and failed to respond to conventional antibiotic treatment. Burkholderia pseudomallei pneumonia with sepsis and necrotizing lymphadenitis were considered by ultrasound-guided hilar lymph node puncture, and the main treatment was to maintain organ function and anti-infection such as Imipenem. The patient was discharged from the hospital without complaining of discomfort, and there were no symptoms such as fever again during later follow-up. Combined with the problems in treatment, this article reviewed the relevant literature, and reported the experience in the diagnosis and treatment of Burkholderia pseudomallei pneumonia with sepsis and necrotizing lymphadenitis. The clinical diagnosis of melioidosis is very challenging and easy to be misdiagnosed or missed. In clinical practice, when the diagnosis of related infectious diseases is not clear, the possibility of Burkholderia pseudomallei infection should be taken into account in time, and the importance of early diagnosis and treatment of melioidosis should be clarified for clinical workers to avoid misdiagnosis or missed diagnosis.

As a passive monitoring system, most of the infectious disease monitoring systems are based on the medical record information after diagnosis, and lack of early warning of new infectious diseases, a monitoring and early warning system for emerging infectious diseases based on the clinical real world data of the whole medical cycle is built. Based on the intelligent system establish by the hospital before, during, and after diagnosis, collect the clinical real world data of the whole medical cycle; combined with Bayesian network and W&D algorithm, the early warning model of infectious disease is constructed, and the knowledge map of infectious disease is built. Establish silent monitoring early warning and active sniffing early warning based on the intelligent diagnosis model of infectious diseases and confirm with experts; establish a comprehensive analysis and early warning center. The results show that the system can realize the monitoring and early warning of emerging infectious diseases from the two perspectives of silent monitoring and active sniffing, which is of great significance for improving the efficiency and effect of epidemic prevention and control, and making the prevention and control threshold move forward.

In this paper, the PDCA cycle management research method is used to improve and study the performance assessment of large-scale activities in clinical departments of hospitals in detail, so as to better improve management work, enhance the comprehensive strength of hospitals, enhance the efficiency of hospital management services, and improve staff satisfaction. Through formulating plans, clarifying problems, improving research, organizing implementation, supervising and summarizing, regular assessment, effective communication and continuous improvement, and constantly improving performance appraisal management methods for large-scale activities, the clinical departments have increasingly strengthened their attention, further improved the management level of functional departments, and better reflected fairness and justice. At the same time, the establishment of a scientific and effective performance appraisal system has aroused the enthusiasm of work, improved the efficiency and quality of work, and achieved good social benefits.
