Burkholderia pseudomallei pneumonia with sepsis and necrotizing lymphadenitis: a case report and literature review |

Abstract Melioidosis is a disease caused by the aerobic Gram-negative bacillus Burkholderia pseudomallei, it has a variety of clinical manifestations, of which Burkholderia pseudomallei is the most serious. A case of recurrent fever, cough, and expectoration was presented in the First Affiliated Hospital of Guangdong Medical University. The patient had repeated symptoms and failed to respond to conventional antibiotic treatment. Burkholderia pseudomallei pneumonia with sepsis and necrotizing lymphadenitis were considered by ultrasound-guided hilar lymph node puncture, and the main treatment was to maintain organ function and anti-infection such as Imipenem. The patient was discharged from the hospital without complaining of discomfort, and there were no symptoms such as fever again during later follow-up. Combined with the problems in treatment, this article reviewed the relevant literature, and reported the experience in the diagnosis and treatment of Burkholderia pseudomallei pneumonia with sepsis and necrotizing lymphadenitis. The clinical diagnosis of melioidosis is very challenging and easy to be misdiagnosed or missed. In clinical practice, when the diagnosis of related infectious diseases is not clear, the possibility of Burkholderia pseudomallei infection should be taken into account in time, and the importance of early diagnosis and treatment of melioidosis should be clarified for clinical workers to avoid misdiagnosis or missed diagnosis.
