4 Medication rule of Professor Lü Wenliang in treating autoimmune liver disease based on data mining

Objective To explore the medication rules and academic thoughts of Professor Lü Wenliang’s treatment of autoimmune liver disease, through sorting out the medical records of Professor Lü Wenliang’s treatment of autoimmune liver disease, based on the data mining. Methods The outpatient medical record information of Professor Lü Wenliang from January 1, 2019 to January 1, 2022 was retrieved through the Information Center of Guang’anmen Hospital, China Academy of Chinese Medical Sciences, the patients diagnosed with autoimmune liver disease by Western medicine were screened, and the information of diagnosis and prescription of traditional Chinese and western medicine was extracted. Excel 2016 was used to standardize the prescription information into a database. Traditional Chinese medicine inheritance computing platform V 3.0 and R language (R 4.2.1) were used to calculate the distribution rules of drug use frequency, efficacy, four qi, five flavor, and meridian conversion, then the rule of formulation and cluster analysis were carried out. Results A total of 658 prescriptions and 158 traditional Chinese medicines were sorted out by inclusion and exclusion standard screening. The analysis of the rule of drug use showed that the four qi of the traditional Chinese medicine were mainly warm, cold, and flat; the five flavors are mainly bitter, sweet, and hot, the main channels are liver, spleen, lung, and stomach. The drug effect is mainly to supplement deficiency class, clearing heat class, and digestion class. In terms of the frequency of drug use, the top five drugs were Astragalus, Paeony, Hawthorn, Scutellaria, and Coptis chinensis. Association rule analysis showed that Astragalus-Paeony was the most frequently used antidrug. The prescriptions can be divided into four clusters by K-means clustering algorithm, and the core prescription drugs can be obtained. Conclusion Professor Lü Wenliang’s prescription for the treatment of autoimmune liver disease has distinct medication rules. The treatment is based on the principle of “invigorating qi and blood, softening the liver and strengthening the spleen, clearing heat and removing dampness”. The medication pays attention to the harmonization of qi and blood, matching cold and temperature, lifting and lowering of qi and supplementing both. It provides a reference for the diagnosis and treatment of autoimmune liver diseases.

12 Study of prognostic model for thyroid cancer based on pyroptosis-related lncRNA#br#

Objective To construct pyroptosis-related long non-coding RNA(lncRNA) prognostic model for thyroid cancer and explore its prognostic value. Methods The genes related to pyroptosis were obtained from MSigDB database; the sequencing data and clinical data of thyroid cancer patients were obtained from the Cancer Genome Atlas database and they were divided into experimental group and validation group at a ratio of 1∶1; the co-expression network of pyroptosis-related genes and lncRNAs was constructed, and the correction coefficient |r|>0.5 and P<0.001 lncRNAs were screened. Univariate Cox regression analysis identified lncRNAs associated with prognosis. Lasso-Cox regression analysis was used to construct a prognostic model, and the risk scores of experimental group and validation group were calculated. Thyroid cancer patients were divided into high-risk and low-risk groups according to the optimal cut-off value of the receiver operating characteristics (ROC) curve of the risk score. Survival analysis was performed using Kaplan-Meier method. Univariate and multivariate Cox regression analyses were used to evaluated the independent prognostic value of risk scores. Immune infiltration within the tumor was analyzed by CIBERSORT and the correlation between risk score and immune checkpoint was analyzed. Results In this study, a total of 1 035 lncRNAs associated with pyroptosis were obtained, and 10 lncRNAs were finally screened. Risk score =1.197×AC092809.3+0.285×MIR3945HG+1.886×AL450326.1+1.457×AL360181.2+0.539×LMNTD2-AS1+1.090×AC069360.1-0.709×DOCK9-DT +0.841×GAPLINC+1.013×PARAL1+0.870×SLC12A5-AS1, the optimal cut-off value obtained by ROC curve was 0.050. The survival rate of high-risk group was lower than that of low-risk group (P<0.01). Univariate and multivariate Cox regression analysis showed that the HR values of risk scores were 2.718 (95%CI: 2.039- 3.624, P<0.01) and 4.832 (95%CI: 1.562- 14.945, P<0.01), respectively. In high-risk group, monocytes, memory B cells, and eosinophils increased, and mast cells and regulatory T cells decreased. Risk scores were associated with expression of CD80, CD160, and ADORA2A at immune checkpoints. Conclusion The prognostic-related lncRNAs prognostic model constructed in this study has good predictive value for the prognosis of thyroid cancer.

18 Analysis of medication rule of Professor Wang Yaoxian in the treatment IgA nephropathy based on data mining

Objective To explore the medication rule of Professor Wang Yaoxian in the treatment of IgA nephropathy based on the prescription data of Professor Wang Yaoxian’s clinical outpatient cases. Methods With the prescription of the outpatient patients of Professor Wang Yaoxian as the data source from September 2018 to October 2021, the nature flavour, and channel trepism analysis, frequency statistical analysis, association analysis, and cluster analysis of the drugs were carried out to explore the law of drug application. Results There were 93 prescriptions that met the inclusion criteria, with a total of 160 herbs. The most common nature of these drugs was cold, the most common flavors was sweet, and the most common channel tropism was liver meridians. Among them, astragalus, turtle shells, and soil turtle insects were most frequently used. The common drug couples were seaweed and turtle shell, peach kernel and leech. The common collocation principles were mainly to activate blood and remove blood stasis, eliminate mass and dissipate knots, invigorate the spleen, eliminate wind and diuresis, invigorate the kidney and activate blood, dispel wind, clear heat and disperse dampness, and activate blood and cool blood. Conclusion Professor Wang Yaoxian in the treatment of IgA nephropathy is combined with the theory of mechanism differentiation, which is often combined with drugs such as supplementing qi and strengthening vital qi, dispelling wind and eliminating dumpness, promoting blood circulation and removing blood stasis and clearing collagals, eliminating disease and dispersing knot.

22 Identify biomolecular markers and targeted drugs for pancreatic ductal adenocarcinoma based on autophagy- long non-coding RNA  regulatory networks

Objective To systematically explore the long non-coding RNA (lncRNA) related to the prognosis of pancreatic ductal adenocarcinoma (PDAC) as well as the potential therapeutic drugs by using public transcriptome RNA sequencing data. Methods Transcriptome (RNA-seq) and clinical data for PDAC patients were obtained from TCGA database. The autophagy-related lnc RNA(ARlncRNA) gene expressions were identified by pearson correlation analysis, than filtered by univariate Kaplan-Meier method and multivariate Cox regression analysis, the gene network regulated by these ARlncRNA was constructed. Finally, the target drugs recognitied by the protein-protein interaction network and Drug-Gene Interaction database (DGIdbs). Results A total of 12 ARlncRNA related to the prognosis of the PDAC patients, the ten coexpressed mRNA (core genes) had significant influence on the overall survival of patients. Enrichment analysis mainly provided the autophagy pathway. Network analysis and database mining identified three potential drugs for PDAC(Afatinib, Dacomitinib, and Osimertinib). Conclusion The ARlncRNA-mRNA network is established to reveal the pathogenesis of PDAC and identify molecular markers, the three potential therapeutic drugs for PDAC are identified by DGIdbs.

27 Exploration of the mechanism of Bushen Bitong Prescription in the treatment of knee osteoarthritis based on network pharmacology and experimental verification

Objective To explore the mechanism of Bushen Bitong Prescription in the treatment of knee osteoarthritis (KOA) by network pharmacology and experimental verification. Methods Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform were used to obtain the active ingredients and targets of Bushen Bitong Prescription. GeneCards, OMIM, PharmGkb, TTD, and DrugBank databases were used to search KOA disease related targets, and the intersection of drug and disease targets was selected. The obtained active ingredients and related targets were introduced into Cytoscape software, and the network map of “Bushen Bitong Prescription - active ingredients-KOA targets” was drawn to screen the key active ingredients. Potential targets were imported into the STRING database to plot the protein-protein interaction network. The core targets of Bushen Bitong Prescription in the treatment of KOA were screened, and GO enrichment and KEGG annotation were analyzed. Human chondrocytes were selected and set up in control group, injury group, and administration group. The control group was cultured normally, the injury group and administration group were cultured in the medium containing interleukin (IL)-1β (10 ng/ml) DMEM /F12, and the administration group was treated with 0.444 7 μg/ml Bushen Bitong Prescription liquid. After 24 h of culture, the cell morphology of the three groups was observed. The levels of inflammatory factors were detected by enzyme-linked immunosorbent assay, and the expression of cartilage repair factors in the three groups was detected by qRT-PCR and Western blot. Results A total of 186 active ingredients of Bushen Bitong Prescription were screened, mainly including quercetin, luteolin, kaempferol, etc., mainly through the key targets of TNF, MMP1, CXCL2, IL1B, and signaling pathways of PI3K/Akt, T cell receptors, toll-like receptors, hypoxy-inducer factor-1, TNF, IL-17, and others. It involved, oxidative stress, apoptosis, inflammation, and other biological processes. Microscopically, the number of cells decreased in the injury group and increased in the administration group. The levels of tumor necrosis factor-α (TNF-α), IL-1β, and IL-6 in injury group were higher than those in control group, and the levels of transforming growth factor-β (TGF-β), insulin-like growth factor-1 (IGF-1), basic fibroblast growth factor (BFGF), and the gene and protein expression levels of SOX9 and COL2A1 were lower than those in control group (P<0.05). The levels of TNF-α, IL-1β, and IL-6 in administration group were lower than those in injury group, and the levels of TGF-β, IGF-1, BFGF, and the gene and protein expression levels of SOX9 and COL2A1 were higher than those in injury group (P<0.05). Conclusion Bushen Bitong Prescription may promote the repair of injured chondrocytes by relieving inflammatory cell infiltration and participating in the regulation of SOX9 and COL2A1.

35 Effects of Yixin Dingji Decoction on microRNA-1 and its regulatory protein connexin43 in rats with ischemic arrhythmia

Objective To explore the effects of Yixin Dingji Decoction on microRNA-1 (miRNA-1) and its regulatory protein connexin43 (Cx43) in rats with ischemic arrhythmia. Methods Sixty SPF 10-week-old male SD rats with a body weight of (250±20) g were divided into six groups by random number table method: sham operation group, model group, Propranolol control group, low, medium, and high dose groups of Yixin Dingji Decoction, with ten rats in each group. After feeding the animals for one week, gavage was administered. Low, medium, and high dose groups of Yixin Dingji Decoction were given 13.95, 27.90, 55.80 g/(kg·d) Yixin Dingji Decoction. Propranolol control group was given Propranolol (9.5 mg/7 ml) by gavage of 5 mg/kg a day. Model group and sham operation group were given the same amount of normal saline intragastric administration, one time a day. Each group was treated for two weeks. Except for sham operation group, the other groups were constructed immediately after the end of gavage, and the ischemic arrhythmia model of rats was established by ligation of the anterior descending branch of the left coronary artery. Sham operation group only thoracotomy without ligation. After modeling, the myocardial tissue of rats was collected, and the expression levels of miRNA-1 and Cx43 as well as the phosphorylation levels of Cx43 at Ser262 and Ser368 were detected by quantitative reverse transcription polymerase chain reaction and Western blot. Results Compared with sham operation group, the expression of miRNA-1 was increased and Cx43 was decreased in model group (P<0.05). Compared with model group, miRNA-1 expression was decreased in low, medium, and high dose groups of Yixin Dingji Decoction, while Cx43 expression was increased in medium and high dose groups of Yixin Dingji Decoction (P<0.05). Compared with low and medium dose groups of Yixin Dingji Decoction, miRNA-1 expression was decreased and Cx43 expression was increased in high dose group of Yixin Dingji Decoction (P<0.05). Compared with the sham operation group, the phosp- horylation of Cx43 at sites Ser262 and Ser368 was decreased in the model group (P<0.05). Compared with model group, the phosphorylation of Cx43 at sites Ser262 and Ser368 in high dose group of Yixin Dingji Decoction was increased (P<0.05). Compared with low dose group of Yixin Dingji Decoction, the phosphorylation of Cx43 at sites Ser262 and Ser368 in high dose group of Yixin Dingji Decoction was increased (P<0.05). The phosphorylation of Cx43 at sites Ser262 and Ser368 in high dose group of Yixin Dingji Decoction was not significantly different from that in  medium dose group of Yixin Dingji Decoction (P>0.05). Hematoxylin and eosin results showed that high dose group of Yixin Dingji Decoction could alleviate myocardial cell injury caused by ischemia reperfusion. Conclusion Yixin Dingji Decoction may effectively correct the expression of Cx43 by down-regulating the expression of miRNA-1 in myocardial tissue and weakening the post-transcriptional inhibition of Cx43. The pharmacology of Yixin Dingji Decoction is preliminarily explored in this study, which provided theoretical support for the effective prevention and treatment of ischemic arrhythmia.

Cardiovascular disease is a world public health problem and one of the main causes of human death. Therapeutic angiogenesis can effectively alleviate myocardial ischemia injury and improve cardiac function, which has become an important supplementary treatment strategy after myocardial ischemia. The regulation mechanism of pericyte-endothelial cell Crosstalk on micro angiogenesis after myocardial ischemia mainly includes pericyte contact with endothelial cells through vascular basement membrane and paracrine regulation. Paracrine regulation plays an important role in stabilizing blood vessel/blood system, and improving cardiac function by promoting angiogenesis cytokines, and activating various signal pathways to promote angiogenesis and maturation. Therefore, pericyte-endothelial cell Crosstalk is the key link of micro angiogenesis after myocardial ischemia. This paper reviews the mechanism and role of pericyte-endothelial cell Crosstalk in micro angiogenesis after myocardial ischemia in order to provide ideas for complementary treatment of myocardial ischemia.

45 Progress in the application of zebra fish model in cancer research

Searching for a reasonable and effective tumor model is crucial for tumor research. In recent years, zebra fish has been favored by researchers because of its high genetic similarity with human, transparent embryos and larvae, strong reproductive ability, short research period, and low cost, and has been used in the research of various tumor models such as colorectal cancer, liver cancer, and cervical cancer. This paper summarizes the recently published zebra fish tumor models, clarifies their construction methods, digs deep into their application value, explains their pros and cons, and briefly discusses on the prospects of zebra fish in tumor prevention and treatment, fundamentally expects to provide a reference for the application of zebra fish tumor models.

Acute lymphoblastic leukemia (ALL) can occur at all ages, which is a malignant tumor in the human blood system and is often accompanied by symptoms such as fever, bleeding, anemia, and organ damage. In recent years, traditional Chinese medicine (TCM) has made some progress in treating ALL. This article will systematically summarize the current situation of TCM against ALL based on the perspective of clinical dialectical classification and molecular biological mechanism, aiming to reveal the potential advantages of TCM including reducing chemotherapeutic untoward effects, reducing relapse of drug resistance, inducing cell apoptosis and autophagy, inhibiting cell proliferation and tumor invasion and metastasis, reversing multidrug resistance, and reduce toxicity and increasing efficiency. It will also clarify the unique efficacy of TCM in effectively prolonging the life of ALL patients and reducing relapse, so as to provide enlightenment and new ideas for clinical treatment of ALL in the future.

53 Research progress on the molecular mechanism of somatostatin in the treatment of obesity

The prevalence of obesity is increasing year by year, and obesity is a potential risk factor for many chronic diseases. However, obesity treatment faces problems such as few drug options and many side effects. Somatostatin (SST) plays an important role in food intake and appetite regulation. More and more evidence suggests that SST has potential value of clinical application in the management of obesity by affecting satiety and feeding behavior. SST can effectively regulate the occurrence and development of obesity and its related diseases by forming a wide regulatory network with insulin, agouti-related peptide, proopiomelanocortin, leptin, ghrelin, and brain-derived neurotrophic factor. The in-depth study of the role and mechanism of SST in weight loss will open up a new perspective for the development of new drugs for the treatment of obesity. In addition, considering the limitations of natural hormones, SST analogs are good candidates for the treatment of obesity and are expected to be widely developed for clinical application. According to the existing problems, this paper also proposes specific research directions in the future, aiming at providing relevant scientific basis for the treatment of obesity.

57 Research progress of exosomes in immune response to gastric cancer

Exosomes are bioactive lipid bilayer nanoscale vesicles secreted by various cells that mediate cell signaling. Exosomes can carry immunosuppressive molecules and cytokines known to interfere with the function of immune cells and act on target cells, directly or indirectly affecting the development, maturation, and antitumor activity of immune cells, thereby inducing immunosuppression of tumors. Exosomes derives from gastric cancer cells and immune cells can be used as regulators of tumor immunity to induce tumor immune escape as communication mediators to promote the progression of gastric cancer. In this review, the research status and progress of exosomes from gastric cancer cells, and different immune cells in the regulation of gastric cancer immune response are reviewed.

61 Establishment of scoring criteria and empirical study of assessment index system on the capacity of clinical medical postgraduate tutors

Objective To formulate the index connotation and scoring standard of the evaluation index system on the capacity of clinical medical postgraduate tutors, and to carry out empirical test. Methods “Delphi Method” was used to carry out expert correspondence consultation and clarify the index connotation and scoring criteria of the index system. The graduate supervisor ability of nine graduate supervisors in the Department of Internal Medicine of Beijing Tiantan Hospital in 2019 was taken as the analysis object, a total of 27 third-level indexes of supervisor ability were scored. Results The connotation and grading standards of 27 third-level indexes of the index system are determined. The average score of the ability evaluation of post graduate tutors was 64.36 points, and the average score rate of the first-level indexex “the situation of graduate students supervised” and “the personal situation of tutors” were 74.60% and 54.11%, respectively. The average score rate of second-level indexes: 93.75% for “ideological and moral education of graduate students”, 85.71% for graduate research and clinical studies, 74.67% for “ethics and ethics of teachers”, 33.56% for “academic research of supervisors”, 33.08% for “comprehensive quality and employment of graduate students”. Conclusion The index system provides the basis of empirical research for the ability evaluation of clinical medical graduate tutors and has good application value.

65 Analysis of factors influencing compliance of patients with chronic subjective tinnitus treated with multiple complex sounds

Objective To explore the factors influencing the compliance of chronic subjective tinnitus patients treated with multiple complex sound therapy. Methods The clinical data of 106 patients with chronic subjective tinnitus treated with multiple compound sound were retrospectively analyzed from January 2020 to December 2021 in the Department of Otolaryngology and Head and Neck, Tangdu Hospital, the Second Affiliated Hospital of Air Force Military Medical University. The self-made compliance questionnaire was used to score, and the patient with compliance score ≥30 points were included in the good compliance group (63 cases), and the poor compliance group (43 cases). Univariate and multivariate logistic regression were used to analyze the related factors affecting treatment compliance. Results There were statistically significant differences in age, course of disease, educational level, marital status, monthly income, adverse reactions, disease cognition, payment method, and social support between the two groups (P<0.05). Logistic regression results showed that monthly income, adverse reactions, disease cognition, and payment method were independent risk factors affecting the compliance of patients with chronic subjective tinnitus with multiple complex sound therapy (OR>1, P<0.05). Conclusion In the treatment of chronic subjective tinnitus with multiple complex sounds, patients have serious adverse reactions, poor disease cognition, low income level, and out-of-pocket expenses, which may reduce treatment compliance.

69 Medical equipment quality control management based on PDCA framework

At present, with the progress of national medical reform, hospitals gradually pay attention to the normalization, standardized, and refined management of medical equipment. Cangzhou People’s Hospital of Hebei Province has formulated a medical equipment quality control program that conforms to the basic conditions of the hospital, the medical equipment quality control management in some departments of the hospital was investigated by random sampling and statistical analysis. It is found that the quality control inspection results are lower than the quality control target, the real reason for the low result was found out by fishbone diagram, through Daming circle (also known as PDCA cycle) actively developed the medical equipment quality control management measures and improvement methods, so that the hospital medical equipment quality control management has been improved.

Objective To explore the influence and predictive value of homocysteine (Hcy), red blood cell volume distribution width (RDW), and glycosylated hemoglobin variation index (HGI) on the occurrence of diabetes foot (DF). Methods A total of 166 patients with type 2 diabetes mellitus (T2DM) admitted to the Department of Endocrinology, the Second Affiliated Hospital of Xi’an Jiaotong University from June 2018 to June 2020 were selected and divided into T2DM group (104 cases) and DF group (62 cases) according to whether they had DF or not. Clinical data, Hcy, and RDW levels of the two groups were collected, and HGI was calculated. The influencing factors of DF occurrence were analyzed by binary logistic regression model, receiver operating characteristic (ROC) curve was drawn and area under the curve (AUC) was calculated, and the predictive index and their value of DF were analyzed. Results Age, glycosylated hemoglobin, fasting blood glucose (FBG), Hcy, RDW, and HGI in DF group were higher than those in T2DM group, and the differences were statistically significant (P<0.05). The results of multivariate analysis showed that FBG, Hcy, RDW, and HGI were independent influencing factors of DF occurrence (P<0.05). The ROC curve analysis results showed that FBG, Hcy, RDW, and HGI had certain predictive value for DF alone and in combination, the AUC of combination of each index was higher than that of each single prediction (P<0.05). Conclusion The combined prediction of FBG, Hcy, RDW, and HGI can be an effective predictor of DF treatment and nursing intervention in T2DM patients.

Objective To explore the efficacy of Shenqi Shiyiwei Granules combined with Levothyroxine Sodium Tablets in the treatment of Hashimoto thyroiditis (HT) with syndrome of yang deficiency of spleen and kidney. Methods A total of 90 HT patients with syndrome of yang deficiency of spleen and kidney admitted to the First Affiliated Hospital of Anhui Medical University from April 2020 to October 2021 were selected. Patients were divided into control 1, 2 groups, and treatment group according to random number table method, with 30 cases in each group. Control 1 group was given Levothyroxine Sodium Tablets, control 2 group was given Shenqi Shiyiwei Granules, treatment group was given Levothyroxine Sodium Tablets combined with Shenqi Shiyiwei Granules. The treatment course of the three groups was three months. The clinical efficacy and traditional Chinese medicine symptom scores, thyroid hormone indexes, thyroid antibody indexes, inflammatory factors, and lymphocyte subsets before and after treatment were compared among the three groups. Results The clinical efficacy of the treatment group was better than that of control 1 and 2 groups (P<0.05). After treatment, traditional Chinese medicine symptom scores, anti-thyroglobulin antibody, anti-thyroid peroxidase antibody, thyrotropin hormone, and interleukin-6 levels in the three groups were lower than those before treatment, and those in treatment group were lower than those in control 1 and 2 groups (P<0.05). After treatment, the CD4+/CD8+ levels in three groups were higher than those before treatment (P<0.05). After treatment, the free triiodothyronine, free thyroid hormone, CD3+, CD4+, CD8+, and interleukin-10 levels in three groups were higher than those before treatment, and those in treatment group were higher than those in control 1 and 2 groups (P<0.05). Conclusion Shenqi Shiyiwei Granules combined with Levothyroxine Sodium Tablets have significant effects in the treatment of HT with syndrome of yang deficiency of spleen and kidney, which can improve symptoms, reduce inflammatory factors, and maintain the stability of immune cells.

82 Analysis of influencing factors on postoperative functional rehabilitation of PFNA in the treatment of elderly femoral intertrochanteric fracture

Objective To explore the influencing factors of postoperative functional rehabilitation of proximal femoral nail antirotation (PFNA) in the treatment of elderly femoral intertrochanteric fracture. Methods The clinical data of 220 elderly femoral intertrochanteric fracture who received PFNA surgery in the Third Affiliated Hospital of Beijing University of Chinese Medicine from January 2016 to October 2021 were retrospectively analyzed. The patients were grouped according to the Harris hip score half a year after surgery, in which score ≥80 was divided into the excellent efficacy group and score <80 was divided into the average efficacy group. Univariate analysis was performed on gender, age, fracture type, operation time, operation timing, body mass index, and American Society of Aneshesiologists (ASA) grading of the two groups, and multivariate regression analysis was performed on statistically significant indicators, in order to clarify the influencing factors of functional rehabilitation after PFNA treatment of elderly femoral intertrochanteric fracture. Results Univariate analysis showed that there were no significant differences in gender, operation time, and body mass index between the two groups (P>0.05), but there were significant differences in age, fracture type, operation time, and ASA classification between the two groups (P<0.05). Logistic multiple linear regression analysis showed that age and operation timing were independent factors affecting functional recovery after PFNA treatment of elderly femoral intertrochanteric fracture (P<0.05). Conclusion Advanced age and delayed surgery for various reasons will affect the postoperative functional rehabilitation of PFNA in the treatment of elderly femoral intertrochanteric fractures. Therefore, early conditions should be created for the above patients to undergo surgery to improve clinical efficacy.

86 Effect of two bone implants fusion and internal fixation in the treatment of osteoporotic elderly lumbar spinal stenosis

Objective To observe the effect of PLIF surgery with bone graft alone and fusion apparatus in the treatment of osteopolitic senile degenerative lumbar spinal stenosis. Methods A total of 75 elderly patients with osteoporosis and degenerative lumbar spinal stenosis treated in the Affiliated Hospital of Hebei University of Technology from August 2019 to December 2021 were selected as subjects. By random number table method, they were divided into bone graft treatment group (38 cases) and interbody fusion cage treatment group (37 cases). Visual analog scale (VAS) and Oswestry disability index (ODI) scale were used to score and compare the two groups before and six months after surgery. The height of decompression intervertebral space before surgery, 1 month after surgery and 18 months after surgery, the length of hospitalization, hospitalization cost, and the amount of bone graft in the intervertebral space during decompression between the two groups, and the situation of bone graft fusion between the two groups 3 and 18 months after surgery were compared. Results After operation, ODI and VAS scores of the two groups were lower than those before operation, and the differences were statistically significant (P<0.05). There were no significant differences in postoperative ODI and VAS scores between the two groups (P>0.05). The overall analysis showed that there was no significant difference in the interaction between the two groups (P>0.05). The difference was statistically significant at different time points (P<0.05). Further pair-wise comparison showed that the decompression level vertebral space height of the two groups was higher than that before surgery a month after surgery, and the decompression level vertebral space height of the two groups was lower than that 1 month after surgery and 18 months after surgery, with statistical significance (P<0.05). There was no significant difference in the length of hospitalization between the two groups (P>0.05). The hospitalization cost of the interbody fusion apparatus treatment group was higher than that of the single bone graft treatment group, the amount of bone graft in the decompression intervertebral space was lower than that of the single bone graft treatment group, and the differences were statistically significant (P<0.05). Three months after surgery, the number of bone graft fusion cases in the simple bone graft treatment group was larger than that in the interbody fusion device treatment group, and the difference was statistically significant (P<0.05). There was no significant difference in the number of bone graft fusion cases between the two groups 18 months after surgery (P>0.05). Conclusion For elderly patients with osteoporotic degenerative lumbar spinal stenosis who have failed conservative treatment, posterior lumbar spinal decompression, simple bone graft fusion, and pedicle screw internal fixation can achieve economical and effective treatment.

90 Clinical effect of extended modified posteromedial approach and posterolateral approach for open reduction and internal fixation of Pilon fractures

Objective To explore the clinical effect of extended modified posteromedial approach and posterolateral approach for open reduction and internal fixation of Pilon fractures. Methods A total of 84 patients with posterior Pilon fracture admitted to Suqian First People’s Hospital from May 2018 to May 2021 were selected and they were divided into the extended modified posterior medial approach group (42 cases) and the posterior lateral approach group (42 cases) according to the random number table method. The operation time, intraoperative blood loss, hospitalization time, weight-bearing time, fracture healing time, American Foot and Ankle Surgery Association (AOFAS) ankle-hindfoot score, visual analog scale (VAS) score, ankle range of motion, Burrwell-Charnley radiology score, and complication rates were compared in both groups. Results The amount of intraoperative bleeding in the extending the modified posterolateral approach group was less than that in the extending the modified posterolateral approach group(P<0.05), and the hospitalization time, weight bearing time, and fracture healing time in the extending the modified posterolateral approach group were shorter than those in the extending the modified posterolateral approach group (P<0.05); at six months after operation, the AOFAS scores of the two groups were higher those at three months after operation, and the prolonged modified posteromedial approach group were higher than the posterolateral approach group (P<0.05); at six months after operation, the VAS scores in both groups were lower than those at three months after operation, and the extended modified posteromedial approach group was lower than that in the posterolateral approach group (P<0.05). At six months after operation, the range of motion of ankle dorsiflexion and plantar flexion in both groups were higher than those at three months after operation, and the extended and modified posteromedial approach group was higher than the posterolateral approach group (P<0.05). The excellent and good rate of the extended and modified posteromedial approach group was higher than that of the posterolateral approach group (P<0.05). There was no significant difference in the incidence of complications between the two groups (P>0.05). Conclusion Compared with the posterolateral approach, extending the modified posteromedial approach can accelerate fracture healing, improve ankle function, relieve postoperative pain, and improve fracture healing, which is safe and reliable.

95 Influencing factors and intervention strategies of kinetophobia in elderly orthopedic surgery patients based on logistic regression

Objective To explore the influencing factors and intervention strategies of kinetophobia in elderly orthopedic surgery patients based on logistic regression analysis. Methods A total of 80 elderly orthopedic surgery patients admitted to wuhu First People’s Hospital, Anhui Province from July 2021 to April 2022 were selected. The patients’ kinetophobia was evaluated by using the Tampa scale of kinesiophobia (TSK) scale after surgery. The patients were divided into the fear of movement group (24 cases with TSK score ≥38 points) and the non-fear group (56 cases with TSK score <38 points). The numerical rating scale (NRS) was used to evaluate patients’ pain and the general self-efficacy scale (GSES) was used to evaluate patients’ self-efficacy. The influencing factors of patients’ fear were determined by single factor comparison and multiple logistic regression analysis. Results There were statistically significant differences in the proportion of gender and education between the two groups (P<0.05). The NRS score of the agnostic group was higher than that of the non-agnostic group, and the GSES score was lower than that of the non-agnostic group, and the differences were statistically significant (P<0.05). Logistic regression results showed that, male (OR [95%CI] =4.158 [1.193-14.491]), education level of junior high school or below (OR [95%CI] =1.942 [1.122-3.361]), NRS score (OR [95%CI] =4.802 [1.162-19.847]) were the risk factors for postoperative motophoresis (P<0.05); GSES score (OR [95%CI] =0.265 [0.092-0.761]) was the protective factor for postoperative moophobia (P<0.05). Conclusion Postoperative rinetophobia in elderly orthopedic surgery patients is related to gender, education level, NRS score and GSES score. Targeted intervention can be conducted according to the above factors to reduce the occurrence of rinetophobia.

99 Expression and clinical significance of Th1/Th2 cytokines in children with respiratory syncytial virus pneumonia

Objective To study the expression and clinical significance of helper T cell (Th)1/Th2 cytokines in children with respiratory syncytial virus pneumonia (RSVP). Methods A total of 117 RSVP children admitted to Children’s Hospital of Hebei Province (hereinafter referred to as “our hospital”) from October 2017 to October 2019 were selected as study group, 117 healthy children in our hospital were selected as control group according to the principle of same sex, age ±2 weeks and 1∶1. According to the severity of the disease, the study group was divided into mild group (73 cases) and moderate and severe group (44 cases). The levels of serum cortisol (Cor), interleukin (IL)-2, interferon -γ (IFN-γ), IL-4, and IL-6 and the expressions of Th1, Th2, and Th1/Th2 were compared among all groups. Results The levels of serum Cor, IL-4, and IL-6 in study group were higher than those in control group, while the levels of IL-2 and IFN-γ were lower than those in control group (P<0.05); the expressions of Th1 and Th1/Th2 in study group were lower than those in control group, while the expression of Th2 was higher than that in control group (P<0.05). The levels of serum Cor, IL-4, and IL-6 in moderate and severe group were higher than those in mild group, while the levels of IL-2 and IFN-γ were lower than those in mild group (P<0.05); the expressions of Th1 and Th1/Th2 in moderate and severe group were lower than those in mild group, while the expression of Th2 was higher than that in mild group (P<0.05). Conclusion RSVP children have Th1/Th2 imbalance and Th2 migration.

102 Risk factors analysis for neurological sequelae of viral encephalitis in children

Objective To explore the risk factors of neurological sequelae of viral encephalitis in children. Methods The clinical data of 80 children with pediatric viral encephalitis who were admitted to Huainan First People’s Hospital, Anhui Province from January 2019 to January 2022 were retrospectively analyzed. The incidence of neurological sequelae in children with viral encephalitis after treatment was counted, the children with neurological sequelae were divided into poor prognosis group and the other children into good prognosis group.The factors affecting the neurological sequelae in children with viral encephalitis after treatment were analyzed. Results Of the 80 children with viral encephalitis, 24 cases had neurological sequelae. There were no significant differences in gender, age, disease course, lesion site, hemiplegia, organ injury, electroencephalogram, serum sodium, and white blood cell count between two groups (P>0.05), the proportion of disorder of consciousness, continuous convulsion, abnormal brain CT, and abnormal brain MRI in poor prognosis group was higher than that in good prognosis group (P<0.05). Disorder of consciousness, continuous convulsion, abnormal brain CT, abnormal brain MRI were risk factors for neurological sequelae in children with viral encephalitis (OR>1, P<0.05). Conclusion Disorder of consciousness, continuous convulsion, abnormal brain CT, and abnormal brain MRI are associated with the neurological sequelae of viral encephalitis.

106 Clinical effect of nasal endoscopic correction of nasal septal deviation combined with inferior turbinoplasty in the treatment of chronic rhinitis with nasal septal deviation

Objective To explore the clinical effect of nasal endoscopic correction of nasal septal deviation combined with inferior turbinoplasty in the treatment of chronic rhinitis with nasal septal deviation. Methods A total of 120 patients with chronic rhinitis with nasal septal deviation treated by 904th Hospital of Joint Logistics Support Force of the Chinese People’s Liberation Army from May 2020 to May 2022 were selected and divided into the control group (receiving nasal endoscopic correction of nasal septal deviation) and the combined group (receiving nasal endoscopic correction of nasal septal deviation combined with inferior turbinoplasty) by random number table method, with 60 cases in each group. Symptom severity score, olfactory recognition threshold, respiratory resistance at 5 Hz oscillation frequency (R5), R20, and quality of life were compared between the two groups before and one month after surgery, and the occurrence of complications in three months after surgery were compared between the two groups. Results One month after surgery, the scores of nasal itching, nasal congestion, sneezing, nasal discharge, and olfactory recognition threshold, R5, R20 in two groups were lower than those before surgery, and those in the combined group were lower than those in the control group, the differences were statistically significant (P<0.05). One month after surgery, scores of nasal symptoms, eye symptoms, other symptoms, behavioral problems, sleep disorders, emotional disorders, and limited activity in both groups were lower than those before surgery, and scores of nasal symptoms, eye symptoms, other symptoms, behavioral problems, and sleep disorders in the combined group were lower than those in the control group, the differences were statistically significant (P<0.05). No nasal septum perforation, cerebrospinal fluid rhinorrhea, abscess or optic nerve injury was found in the two groups three months after surgery. Conclusion Nasal endoscopic correction of nasal septal deviation combined with inferior turbinoplasty is helpful for postoperative recovery of airway function and olfaction of patients, and the surgical safety is high, it is recommended to be widely used in clinic.

110 Equity analysis of public health resource allocation in Inner Mongolia Autonomous Region

Objective To investigate and analyze the distribution and equity of public health resources in Inner Mongolia Autonomous Region in 2020, so as to provide theoretical basis for rational allocation of public health resources under normal epidemic prevention and control. Methods In June 2022, data of public health resources in Inner Mongolia Autonomous Region and the whole country were collected, and the fairness of public health resources allocation was analyzed using Lorentz curve, Gini coefficient, and Theil index were used to analyse. Results In terms of population distribution, the Gini coefficient of various resources in Inner Mongolia Autonomous Region was less than 0.4, and the Theil index of public health technicians was the smallest (0.028 462 957). In terms of geographical area allocation, only the number of public health institutions in eastern Mongolia was less than 0.3, the number of public health technicians, practicing (assistant) physicians and registered nurses were 0.3-0.4, the Gini coefficient of various resources in other regions was more than 0.4, and the Theil index of public health institutions was the smallest (0.433 042 879). Conclusion There is a gap between the growth rate of public health resources in Inner Mongolia Autonomous Region and the national average level. The fairness of population allocation is better than that of geographical area allocation, and the inequality of resource allocation mainly comes from within the region.

Objective To compare the contents of the total flavonoids, total saponins, total polysaccharides of Lianhua Qingwen Capsules from different producing areas and to provide reference for its quality evaluation. Methods  The determination method of total flavonoids, total saponins, and total polysaccharides in Lianhua Qingwen Capsules was established by ultraviolet visible spectrophotometry, and the contents of 12 batches of Lianhua Qingwen Capsules were determined. Results The linear ranges of total flavonoids, total saponins, and total polysaccharides were 200-1 200 μg/ml (r=0.999 9), 20-200 μg/ml (r=0.999 8), 21.36-85.44 μg/ml(r=0.999 7), respectively; the average recoveries were 97.14%, 100.06%, 96.29%, RSD were 0.90%, 0.82%, 1.53%(n=9), respectively; the average mass fraction of total flavonoids, total saponins, and total polysaccharides in 12 batches of samples were 96.87, 163.20, and 200.00 mg/g, respectively. Conclusion The method is with good precision, repeatability and stability, which can be used as a new means for the quality control of Lianhua Qingwen Capsules.

118 Applicability test and result analysis of microbial limit test for Sanqi Powder of traditional Chinese medicine

Objective To establish a microbial limit test and the applicability test for Sanqi Powder from three manufacturers based on Chinese Pharmacopoeia in 2020, and to provide reference for the study of the microbial limit test method of powdered traditional Chinese medicine. Methods According to the four general rules 1108 of Chinese Pharmacopoeia in 2020, the plate method was used to determine the total number of aerobic, mold, and yeast. The applicability of methods for Sabouraud Dextrose Agar, Sabouraud Agar (containing chloramphenicol), and Rose Bengal Agar was studied. The conventional method and dilution method were used to test the applicability of control bacteria. Results The total number of aerobic bacteria in Sanqi Powder from three manufacturers were 1∶100, 1∶50, 1∶20 by plate method, the total number of molds and yeasts were 1∶20, 1∶10, 1∶10 by plate method. Escherichia coli and Gram-negative bacteria were both detected by conventional method, and the maximum volume of Salmonella enrichment liquid tryptophan soybean peptone liquid medium was 200 ml. Conclusion The microbial limit test methods of Sanqi Powder from three manufacturers conforms to the relevant provisions of the fourth general rule of Chinese Pharmacopoeia in 2020, and the method is feasible. Sanqi Powder has a certain inhibitory effect on bacteria. The microbial limit test methods of Sanqi Powder from different manufacturers are not identical.

Objective  To establish a microbial limit test method for Calamine Pigment, Calamine and Sulfur Pigment, and Phenolic Calamine Pigment. Methods According to microbial counting method and control bacteria inspection method of 2020 edition Chinese Pharmacopoeia requirements, the applicability tests were carried out by plate method, medium dilution method, and membrane filtration method. Results The total number of aerobic bacteria: the medium dilution method was used for Calamine Pigment, Calamine and Sulfur Pigment, the membrane filtration method was used for Phenolic Calamine Pigment; the total number of mold and yeasts: the medium dilution method was used for the three preparations; the control bacteria test: the routine method was used for Calamine Pigment, the medium dilution method and routine method were used for Calamine and Sulfur Pigment, the membrane filtration method and routine method were used for Phenolic Calamine Pigment. Conclusion The microbial limit test can meet 2020 edition Chinese Pharmacopoeia requirements, and can be used in the quality evaluation of Calamine Pigment, Calamine and Sulfur Pigment, and Phenolic Calamine Pigment.

Objective To screen the excellent strains through microbial transformation, using 7β,15α-dihydroxy-18β- glycyrrhetinic acid as the target product. Methods Ten fungi (CGMCC 3.149, CGMCC 3.193, CGMCC 3.3402, CGMCC 3.739, CGMCC 3.2462, CGMCC 3.2886, CGMCC 3.3400, CGMCC 3.577, CGMCC 3.970, and Zw-4) were used for preliminary screening and re-screening experiments of transformed strains, the content of 7β,15α-dihydroxy-18β-glycyrrhetinic acid in the converted liquid was determined by high performance liquid chromatography. The conversion rate of 7β,15α-dihydroxy-18β-glycyrrhetinic acid was used as the evaluation index and the strains with high conversion rate and consistent results of preliminary screening and re-screening experiments were selected as the excellent strains. 7β,15α-dihydroxy- 18β-glycyrrhetinic acid showed a good linear relationship in the range of 2.56~81.79 μg/ml. The strain with the highest conversion rate of preliminary screening experiment was CGMCC 3.3400. The strain with the highest conversion rate of re-screening experiment  was CGMCC 3.970. Conclusion The excellent strain is CGMCC 3.970.

131 Study on the influencing factors of Flurbiprofen Axetil related acute kidney injury after surgery

Objective To analyze the influencing factors of Flurbiprofen Axetil (FA) related acute kidney injury (AKI) after surgery. Methods All surgical inpatients who used FA in Zhuzhou Central Hospital between January and December 2019 were retrospectively investigated. The clinical data, use time, dosage and treatment course of FA were collected. The creatinine value of baseline and within seven days after treatment and others were collected. Univariate and multivariate analyses were performed with logistic regression model to assess the risk factors of AKI. Results Among 859 patients who received postoperative FA analgesia, 94 patients developed AKI. The incidence rate of AKI in patients with different ages, severe anemia, hypoproteinemia, chronic kidney disease, liver cirrhosis, and combined use of diuretics, angiotensin Ⅱ receptor blockers (ARB), and angiotensin-converting enzyme inhibitor (ACEI) were compared, with statistical differences (P<0.05). The results of multi factor analysis showed that severe anemia (OR=23.094, 95%CI: 4.604 to 115.853), chronic kidney disease (OR=12.080, 95%CI: 5.505 to 26.509), liver cirrhosis (OR=4.113, 95%CI: 1.602 to 10.558), ARB (OR=6.076, 95%CI: 2.553 to 14.462), and ACEI (OR=5.985, 95%CI: 3.060 to 11.703) were independent risk factors for the occurrence of AKI in patients (P<0.05). Age 19 to 45 years old (OR=0.359, 95%CI: 0.172 to 0.753) and > 45 to 65 years old (OR=0.330, 95%CI: 0.181 to 0.604) were independent protective factors for AKI (P<0.05). Conclusion FA related AKI is common after surgery. Early identification of risk factors and proactive interventions to prevent and reduce the occurrence of AKI.

135 Effect of acupuncture on quality of life in patients with atrial fibrillation after catheter ablation

Objective To investigate the effect of acupuncture on quality of life in patients with atrial fibrillation (atrial fibrillation for short) after catheter ablation.  Methods A total of 60 eligible patients with atrial fibrillation after catheter ablation who were admitted to Dongzhimen Hospital Affiliated to Beijing University of Chinese Medicine, Jiangsu Province Hospital, Peking University People’s Hospital, Peking University Third Hospital, First Teaching Hospital of Tianjin University of Traditional Chinese Medicine, Hebei Traditional Chinese Medicine Hospital, and Renmin Hospital of Wuhan University from May 2021 to February 2022 were selected as subjects and they were divided into acupuncture group and sham acupuncture group according to the central block randomization system, with 30 cases in each group. Acupuncture was applied at Xinshu, Neiguan and Shenmen in the acupuncture group. Three non-meridian and non-acupoint points were treated with shallow acupuncture in the sham acupuncture group. In both groups, acupuncture was performed twice a week from one to six weeks and once a week from 7 to 12 weeks, with a total of 18 times of treatment. The score of the atrial fibrillation effect and quality of life scale (AFEQT) from baseline to three months after catheter ablation, AF burden, recurrence of AF, number of being hospitalized for arrhythmia, and adverse events were compared between the two groups. Results After treatment, the total score of AFEQT, symptoms, daily activities, treatment concerns, atrial fibrillation control, symptom improvement satisfaction scores of the two groups were higher than before treatment (P<0.05), except for daily activities score, there were no statistical significance between the two group (P>0.05). There was no significant difference in the atrial fibrillation load, the recurrence of atrial fibrillation, the number of hospitalizations due to arrhythmia and the incidence of adverse events three months after surgery between the two groups (P>0.05). Conclusion Acupuncture may improve the quality of life of patients with AF after catheter ablation in 3 months. Multi-center, large-sample, randomized controlled trials can be carried out in the near future to verify the efficacy and safety of acupuncture in the treatment of AF.

141 Discussion on Wang Youpeng’s experience in treating children with cough variant asthma in pediatrics of cold regions based on “precision medicine”

Based on “precision medicine”, this paper discusses Professor Wang Youpeng’s experience in treating children with cough variant asthma. Professor Wang blames the etiology and pathogenesis of cough variant asthma in children on climate, constitution, allergen, and other factors, innovatively put forward the theory of “pediatrics in cold regions”. He believes that children in cold regions are mostly characterized by damp heat constitution, and the main pathological factors of cough variant asthma are wind, phlegm, and blood stasis. Clinical syndrome is based on the theory of “collateral disease”, which suggests that the pathogenesis of the disease is characterized by “acute lung collateral deficiency and stagnation of collateral qi” during the onset period, and “deficiency and stagnation of collateral qi, accumulation of collateral qi” during the remission period. Based on the theory of “treating based on evidence”, prevention before disease and prevention of existing diseases and changes. Accurately individualized diagnosis and treatment based on “three differentiation thought” of the premise of disease differentiation, syndrome differentiation as the core, and body differentiation as the key. With the method of “separating dispersion and mobilizing discharge” and taking Wendan Decoction as the main prescription, a series of prescriptions are created, such as Xiebai Wendan Decoction, Xuanbei Wendan Decoction, Conglong Wendan Decoction, Sanzi Wendan Decoction and Baiguo Wendan Decoction. On the basis of accurate syndrome differentiation, precise treatment has been carried out with good results.

146 Clinical effect of ultrasound-guided acupotomy combined with myofascial trigger point of flexor pollicis longus inactivation in treatment of thumb stenotic tenosynovitis

Objective To explore the clinical effect of ultrasound-guided needle-knife combined with myofascial trigger of flexor pollicis longus inactivation (“myofascial trigger point of flexor pollicis” for short)in the treatment of thumb tenosynovitis stenosans. Methods A total of 100 patients with thumb stenotic tenosynovitis treated in the Third Affiliated Hospital of Beijing University of Chinese Medicine from January 2020 to December 2022 were divided into the observation group and the control group according to the random number table method, with 50 cases in each group. The treatment group was treated with ultrasound-guided acupotomy combined with myofascial trigger point of flexor pollicis longus inactivation, and the control group was treated with ultrasound-guided acupotomy.  Visual analogue scale(VAS) and trigger finger scores of the two groups were compared at three different time points before treatment, one week and one month after treatment, the thickness of flexor pollicis longus tendon was measured before treatment and one month after treatment, and the efficacy was evaluated one month after treatment. Results There were statistically significant differences in VAS score, trigger finger score, time point comparison, and interaction between the two groups before treatment, one week and one month after treatment. Further pair comparison, intra-group comparison: VAS score and trigger finger score of both groups were lower than before treatment one week and one month after treatment (P<0.05). Comparison between groups: VAS score and trigger finger score of observation group were lower than those of control group one month after treatment (P<0.05). One month after treatment, the tendon thickness of flexor pollicis longus in both groups was lower than that before treatment, and the observation group was lower than the control group (P<0.05). The clinical effect of observation group was better than control group (P<0.05). Conclusion Ultrasound-guided acupotomy combined with myofascial trigger point of flexor pollicis longus inactivation can effectively improve pain, functional limitation and tendon thickness of flexor pollicis longus in patients with thumb tenosynovitis stenosans.

151 Professor Liu Zhibin’s experience in treating pain by acupuncture picking therapy based on the theory of “taking pain as acupoint”

Pain is a generic term for common clinical pain disorders, which are common and frequent in clinic, and seriously affect patients’ daily life and physical and mental health. At present, analgesic drugs are the main treatment. In clinical application, it is found that such drugs are often ineffective in treating pain caused by osteoarthropathy and chronic intractable diseases. Based on this, Professor Liu Zhibin, a famous traditional Chinese medicine doctor from Shaanxi Province, combining his years of clinical experience and the theory of “taking pain as acupoint”, proposes that the key to treating pain should be “purging blood method to eliminate the long - standing disease in the blood”. In addition, Professor Liu believes that there is a close connection between pain and skin, and according to this proposed “acupuncture picking therapy”, the clinical attention to distinguish acupoints and “taking pain as acupoint” combination, the operation emphasizes the horizontal acupuncture picking, pulling and swaying. This article expatiates on Professor Liu’s experience in treating pain by acupuncture picking therapy from “taking pain as acupoint” and attaches a verified case for discussion.

Amyotrophic lateral sclerosis is a common motor neurone disease in clinical practice, mostly in middle-aged and elderly people, with high disability rate and high mortality. Conventional treatment with western medicine can only delay the progression of the disease in most patients, with poor efficacy and more adverse reactions. The treatment of amyotrophic lateral sclerosis by Professor Lu Fang, a Chinese medical master, has achieved a good effect. Based on the theoretical basis of “dampness-heat leads to atrophy syndrome”, Professor Lu Fang proposes that the core pathological factor of this disease is dampness and heat, and the core disease is located in the spleen, which mainly extends to the three viscera of lung, liver, and kidney. The treatment of “clearing dampness-heat, eliminating and dissipating” runs through the whole course of the disease. Professor Lu Fang takes self-made prescription of Qingjieyin as the basic prescription of the disease, combines with the disease location of the zang-fu viscera, taking into account both deficiency and excess, flexibly used specific specific drugs, and achieved good clinical effect. 159 Clinical effect of warming needle moxibustion combined with Shentong Zhuyu Decoction in the treatment of cervical spondylotic radiculopathy with qi stagnation and blood stasis type

Objective To investigate the clinical effect of warming needle moxibustion combined with Shentong Zhuyu Decoction in the treatment of cervical spondylotic radiculopathy (CSR) with qi stagnation and blood stasis type. Methods A total of 120 patients with CSR admitted to Huizhou Central People’s Hospital of Guangdong Province from May 2020 to May 2022 were selected and divided into acupuncture and moxibustion group, decoction group, and combination group according to random number table method, with 40 cases in each group. The acupuncture and moxibustion group was treated with conventional+warming needle moxibustion, the decoction group was treated with conventional+Shentong Zhuyu Decoction, and the combined group was treated with conventional+warming needle moxibustion+Shentong Zhuyu Decoction. All patients were treated for a course of 30 days and three courses. The traditional Chinese medicine syndrome score, vertebrobasilar artery hemodynamics indexes, clinical efficacy, and treatment safety of the three groups were compared. Results After treatment, the scores of neck pain stiffness, dizziness, upper limb numbness, and fear of cold and cold limbs in the three groups were lower than before treatment, and the combined group was lower than acupuncture and moxibustion group and decoction group (P<0.05). After treatment, the vertebrobasilar artery peak systolic velocity (PSV), end diastolic flow velocity (EDV), and vascular pulsatility index (PI) of the three groups were higher than those before treatment, and the vascular resistance index (RI) of the three groups were lower than before treatment, and the vertebrobasilar artery PSV, EDV, and PI of the combined group were higher than acupuncture and moxibustion group and decoction group, and RI was lower than acupuncture and moxibustion group and decoction group (P<0.05). The curative effect of combination group was better than acupuncture group and decoction group (P<0.05). No serious adverse reactions occurred in the three groups. Conclusion Warming needle moxibustion combined with Shentong Zhuyu Decoction has significant curative effect in the treatment of CSR, which can improve patients’ traditional Chinese medicine symptoms and vertebrobasilar artery hemodynamics.

163 Effects of nursing intervention guided by positive strengthening theory on pain and functional exercise compliance in patients with rheumatoid arthritis

Objective To observe the effects of nursing intervention guided by positive strengthening theory on pain and functional exercise compliance in patients with rheumatoid arthritis (RA). Methods A total of 96 RA patients admitted to the Affiliated Huaian NO.1 Hospital of Nanjing Medical University from February 2018 to February 2020 were selected as subjects by convenience sampling method. According to random number table method, they were divided into control group (49 cases) and observation group (49 cases). The control group received routine nursing, and the observation group received nursing intervention guided by positive strengthening theory on the basis of the control group, the intervention time was eight weeks. Subjective pain, range of motion, and functional exercise compliance were compared between two groups before intervention, and at two and eight weeks of intervention. Results In the observation group, two patients left the group midway, and 47 patients were enrolled at last. The overall analysis showed that there were statistically significant differences in visual analog scale (VAS) scores, disease activity score 28 (DAS28),  and family exercise compliance scale between two groups in intergroup, time, and interaction (P<0.05). Intra-group comparison: VAS scores, DAS28 scores, and family exercise compliance scale scores of control group at different time points showed no significant difference (P>0.05). At two and eight weeks of intervention, VAS scores and DAS28 scores of observation group were lower than those before intervention, and functional exercise family exercise compliance scale scores were higher than those before intervention, and the differences were statistically significant (P<0.05). Comparison between groups: at two and eight weeks of intervention, VAS scores and DAS28 scores of observation group were lower than those of control group, and family exercise compliance scale scores were higher than those of control group, and the differences were statistically significant (P<0.05). Conclusion Nursing intervention guided by positive strengthening theory can reduce patients’ subjective pain perception and disease activity during functional exercise, and improve the compliance of functional exercise in the nursing process of patients with RA.

168 Application value of the health education guidance mode of business process reengineering in the perioperative period of minimally invasive surgery for gallstones

Objective To exploration of the application value of the health education guidance mode of business process reengineering(BPR) in the perioperative period of minimally invasive surgery for gallstones. Methods A total of 91 patients with gallstones admitted to Nanjing Gaochun People’s Hospital from January to November 2021 were selected as the study objects. The patients were divided into control group and observation group by random number table method, with 46 cases in control group and 45 cases in observation group. The control group received routine nursing and health education, and the observation group received routine nursing and BPR health education guidance mode. The perioperative stress reaction and postoperative rehabilitation were observed in the two groups. Results In the control group, one case was eliminated due to the worong indes detection results. The overall analysis showed that there were statistically significant differences in the comparison of heart rate and systolic blood pressure between the two groups, time point and interaction comparison (P<0.05). Further pairwise comparison, intra-group comparison: comparison of heart rate and systolic blood pressure between the two groups at various time points during perioperative period, the differences were statistically significant (P<0.05); comparison between groups: the heart rate and systolic blood pressure in the observation group were lower than those in the control group 1 h before surgery, 15 min during surgery, and 1 h after surgery, the differences were statistically significant (P<0.05). The time of moving out of bed, anal exhaust time, and hospital stay in observation group were shorter than those in control group, the differences were statistically significant (P<0.05). Conclusion BPR health education guidance model can reduce the perioperative stress reaction and accelerate the postoperative physical recovery.

172 Application effect of guided nursing path table in patients with type Stanford B aortic dissection undergoing endovascular isolation

Objective To explore the application effect of guided nursing path table in patients with type Stanford B aortic dissection undergoing endovascular isolation. Methods A total of 98 patients with type Stanford B aortic dissection stent isolation were selected from Jiangsu Provincial People’s Hospital from December 2018 to December 2021. The patients were divided into observation group and control group, with 49 cases in each group. The control group received routine nursing intervention, and the observation group was given a nursing path table with guidance as the target. Operation intraoperative blood loss, drainage time, postoperative recovery time and hospital stay, postoperative adverse events and quality of life were compared between the two groups. Results The intraoperative blood loss, drainage time, postoperative recovery time and hospital stay of the observation group were lower than those of the control group, and the difference were statistically significant (P<0.05); the total incidence of postoperative adverse events in observation group was lower than that in control group, and the difference was statistically significant (P<0.05). Life quality scores after intervention were higher in both groups than before intervention, and observation group was higher than control group, and the differences were statistically significant (P<0.05). Conclusion The guided nursing path table in patients with type Stanford B aortic dissection undergoing endovascular isolation, can improve the quality of life, improve the surgical situation of patients, reduce the overall incidence of adverse events.

176 Application effect of nursing model based on pediatric early warning score in children with high fall injuries

Objective To explore the application effect of nursing model based on pediatric early warning score (PEWS) in children with high fall injuries. Methods A total of 102 children with high fall injuries and 102 family members admitted to Anhui Children’s Hospital from July 2019 to May 2022 were selected as the research objects. The children were numbered according to the admission order, there was one family member for each case, and they were divided into control group and observation group by random number table method. The control group (51 cases, 51 family members) received routine rescue nursing intervention, and the observation group (51 cases, 51 family members) received nursing model based on PEWS intervention. The stay time in department of emergency, total rescue time, rescue effect, and complications were compared between the children of two groups. The self-rating anxiety scale (SAS) scores of family members of the two groups were compared before intervention (when the child was admitted to hospital) and after intervention (on the day after surgery if there was surgery, on the day after discharge if there was no surgery). Results The accuracy of PEWS was 96.08% for children with high fall injuries. The stay time in department of emergency, total rescue time in children of observation group were shorter than those in the children of control group, and the success rate of rescue in children of observation group was higher than that in the children of control group, the differences were statistically significant (P<0.05). After intervention, the SAS scores of family members in two groups were lower than those before intervention, and the observation group was lower than the control group, the differences were statistically significant (P<0.05). The incidence of complications in the children of observation group was lower than that in the children of control group, and the difference was statistically significant (P<0.05). Conclusion The application of nursing model based on PEWS in the early warning of children with high fall injuries showes a high prediction accuracy, which could effectively improve the efficiency and quality of rescue work, help to reduce the anxiety of family members and reduce the complications of children.

180 Application of improved emergency nursing pathway combined with standardized nursing in intravenous thrombolytic therapy of rt-PA in the early stage of acute cerebral infarction

Objective To explore the application effect of improved emergency nursing pathway combined with standardized nursing in intravenous thrombolytic therapy of recombinant tissue-type plasminogen activator (rt-PA) in the early stage of acute cerebral infarction. Methods Eighty patients with acute cerebral infarction who received early intravenous thrombolysis therapy with rt-PA in the Department of Emergency Internal Medicine of Bozhou People’s Hospital in Anhui Province from May 2020 to October 2021 were selected as the research objects. According to random number table methods, the patients were divided into observation group and control group, with 40 cases in each group. The observation group was given improved emergency nursing path combined with standardized nursing, and the control group was given routine emergency nursing. The efficacy of thrombolytic therapy, emergency time, National Institutes of Health stroke scale (NIHSS) score, activity of daily living (ADL) score, complication rate, and patient satisfaction were compared between the two groups. Results The treatment effect of observation group was better than control group (P<0.05). The time of blood sampling, CT examination, and the time from admission to thrombolysis in the observation group were shorter than those in the control group (P<0.05). At discharge, NIHSS score of the two groups was lower than that at admission, ADL score of the two groups were higher than that at admission, and NIHSS score of the observation group was lower than that of the control group, and ADL score of the observation group was higher than that of the control group, and the differences were statistically significant (P<0.05). The total incidence of complications in the observation group was lower than that in the control group (P<0.05). The satisfaction of patients in observation group was better than that in control group (P<0.05). Conclusion The application of improved emergency nursing pathway combined with standardized nursing intervention in patients with acute cerebral infarction is helpful to improve the efficacy of early rt-PA intravenous thrombolysis and emergency efficiency, promote rehabilitation, reduce complications, and the patient satisfaction is high.

Objective To explore the influence of micro-video education based on WeChat platform on preoperative self-efficacy and postpartum rehabilitation of pregnant women with repeat cesarean section. Methods A total of 80 pregnant women with repeat cesarean section admitted to the Bengbu Third People’s Hospital in Anhui Province from January to April 2022 were selected as research objects, and they were divided into control group and observation group by random number table method, with 40 cases in each group. The control group received traditional preoperative health education, and the observation group received preoperative micro-video education based on WeChat platform. Both groups were treated for one week. The psychological state, sleep quality, self-efficacy before and after intervention and postpartum recovery of the two groups were observed. Results After intervention, the scores of self-rating sleep status and anxiety scale in two groups were lower than those before intervention, and those in the observation group were lower than those in control group, the differences were statistically significant (P<0.05). After intervention, the scores of outcome expectation-16, efficacy expectation-16, and childbirth self-efficacy inventory-C32 in two groups were higher than those before intervention, and those in the observation group were higher than those in the control group, the differences were statistically significant (P<0.05). The time of getting out of bed, exhaust, and defecation in the observation group was shorter than those in the control group, and the differences were statistically significant (P<0.05). Conclusion Micro-video education based on WeChat platform intervention can significantly relieve negative emotions, promote self-efficacy of pregnant women with repeat cesarean section, and contribute to postpartum recovery.

188 Study on the treatment structure and burden of medical expenses of solid tumor diseases in tertiary public hospitals under diagnosis-intervention packe payment#br#

Objective To provide a reference basis for optimizing the disease structure, controlling medical expenses, and promoting the balance of medical insurance under diagnosis-intervention packe (DIP) payment by analyzing the treatment structure and burden of medical expenses of solid tumor diseases in tertiary hospitals. Methods Three tertiary medical institutions (Chifeng Municipal Hospital, Affiliated Hospital of Chifeng University, Chifeng Tumor Hospital) were sampled from Chifeng City of Inner Mongolia Autonomous Region by using typical sampling as sample hospitals. The DIP payment data, medical insurance reimbursement data, and medical expense data of solid tumor patients hospitalized by each sample hospital from November 2021 to April 2022 were collected. According to the RW and sub-average medical expenses indicators, it was divided into four categories: dominant disease, potential disease, basic disease, and inferior disease. Disease admission structure and medical expenses burden were analyzed. Results A total of 13 946 solid tumor patients were collected. Three hundred and eighty-seven DIP solid tumor diseases were entered according to the main diagnosis and surgical operation data. Among them, there were 142 dominant diseases, 43 potential diseases, 109 basic diseases, and 93 inferior diseases. The dominant disease of hospital had a surplus of 14.01 yuan per 100 yuan of medical expenses. Potential disease had a loss of 13.28 yuan per 100 yuan of medical expenses. Basic disease had a surplus of 7.96 yuan per 100 yuan of medical expenses. Inferior disease had a loss of 7.91 yuan per 100 yuan of medical expenses. Conclusion Under DIP payment, tertiary medical institutions should be guided by DIP disease data, implement precise policies, and optimize the admission structure of disease types in depth. Based on DIP disease types, carry out refined medical cost control work, reduce the burden of patients’ medical expenses, and maintain the balance of hospital medical insurance revenue and expenditure, so as to promote the reform of medical insurance payment methods and achieve a win-win situation for medicine, insurance and patients.

The work of young cadres in medical research institutes is a basic work to strengthen the construction of the leadership and cadre talent team of medical research institutes in the new era. This paper analyzes the current situation of the work of young cadres in medical research institutes in the new era. There are some problems, such as insufficient attention and imperfect work system and insufficient ability and quality of young cadres themselves. Based on the characteristics of medical research institutes, this paper analyzes the reasons and put forward five measures as follows: improve the ideological understanding and shoulder the political responsibility of the work of young cadres; improve the daily discovery mechanism and reserve the reservoir of outstanding young cadres; perfect training and exercise, and build a training platform for young cadres; perfect the use mechanism and lay the foundation stone for young cadres; Improve the management and supervision mechanism, calibrate the growth direction of young cadres which will strengthen and improve the work of young cadres in medical research institutes, so as to provide reference for medical research institutes to do well in the work of young cadres, and lay a foundation for promoting the high-quality development of various undertakings of medical research institutes.
