Abstract Exosomes are bioactive lipid bilayer nanoscale vesicles secreted by various cells that mediate cell signaling. Exosomes can carry immunosuppressive molecules and cytokines known to interfere with the function of immune cells and act on target cells, directly or indirectly affecting the development, maturation, and antitumor activity of immune cells, thereby inducing immunosuppression of tumors. Exosomes derives from gastric cancer cells and immune cells can be used as regulators of tumor immunity to induce tumor immune escape as communication mediators to promote the progression of gastric cancer. In this review, the research status and progress of exosomes from gastric cancer cells, and different immune cells in the regulation of gastric cancer immune response are reviewed.
