DOI: 10.20047/j.issn1673-7210.2023.05.02
Objective To investigate the protective mechanism of Bakuchiol against Streptozotocin induced diabetic cardiomyopathy in rats. Methods Forty-five 8-week-old SPF male SD rats, weighing 180-220 g, were randomly divided into control group, diabetes mellitus group, and Bakuchiol group, with 15 rats in each group. After four weeks of high sugar and fat diet (HFD), the diabetes mellitus group and Bakuchiol group were injected intraperitoneal with Streptozotocin (50 mg/kg) every day. After four weeks of normal diet, the control group was injected intraperitoneally with the same amount of normal saline every day, all the three groups were injected continuously for three days. One week after intraperitoneal injection of Streptozotocin, the three groups were orally given glucose (2 g/kg) on an empty stomach for 12 h, and blood glucose values were recorded at 0, 30, 60, 90, and 120 min. The diabetes mellitus group and Bakuchiol group were considered to be successful in modeling with random blood glucose ≥16.7 mmol/L, the two groups continued to be fed HFD for eight weeks, and the Bakuchiol group was also given Bakuchiol (60 mg/kg) gavage for eight weeks. The control group continued the normal diet for eight weeks. After the intervention, the cardiac function was detected by small animals ultrasound, the heart tissue was taken for hematoxylin-eosin staining and Masson staining, the myocardial apoptosis index was detected by TUNEL staining, and the expression level of myocardial apoptosis related proteins was detected by Western blotting. Results Hematoxylin-eosin staining and Masson staining showed that the myocardial arrangement was disordered in the diabetes mellitus group and the perivascular collagen fibers increased significantly, while the myocardial disorder was improved in the Bakuchiol group and the perivascular collagen fibers decreased significantly. Left ventricular ejection fraction, left ventricular short axis shortening rate, and Bcl-2 levels in diabetes mellitus group were lower than those in control group, while the area under the blood glucose curve, myocardial apoptosis index, Cleaved caspase-3/caspase-3, and Bax expression level in model group were higher than those in control group, the differences were highly statistically significant (P<0.01). Left ventricular ejection fraction, left ventricular short axis shortening rate in Bakuchiol group were higher than those in diabetes mellitus group, while the area under the blood glucose curve, myocardial apoptosis index, Cleaved caspase-3/caspase-3, Bcl-2, and Bax expression level in Bakuchiol group were lower than those in diabetes mellitus group, the differences were statistically significant (P<0.05 or P<0.01). Conclusion Bakuchiol treatment can Significantly reduce streptozotocin induced diabetic cardiomyopathy in rats, inhibit apoptosis signal, reduce myocardial cell apoptosis, and improve cardiac function.

NIE Lianghui WANG Shuai YANG Libin MA Rong CHEN Hanjiang RONG Ping
DOI: 10.20047/j.issn1673-7210.2023.05.07
Infantile spasms is a typical catastrophic epileptic encephalopathy, most of which are refractory epilepsy. Currently, the pathogenesis of infantile spasms is not fully clearly. In recent years, gene variants such as SCN2A, SCN8A, CDKL5, DEPDC5, STXBP1, GABRA, COL4A1/COL4A2, and CASK etc. have been gradually confirmed to be related to infantile spasms. Due to the specificity of clinical phenotypes caused by different gene mutations, western medicine treatment selects different therapeutic drugs with different targets for different pathogenesis. Traditional Chinese medicine, based on the constitution and disease characteristics of children, can also have certain curative effects by treating according to syndrome differentiation. Therefore, this article reviews the etiology and diagnosis and treatment of infantile spasms gene-related genetics in order to identify relevant gene mutations at an early stage and improve the efficiency of clinical diagnosis and treatment.

DOI: 10.20047/j.issn1673-7210.2023.05.08
Chronic diseases, such as diabetes mellitus, hypertension, cardiovascular diseases, malignant tumors, and chronic respiratory diseases, seriously damage the health of individuals and groups. The composition of human body includes water, protein, body fat, inorganic salt, etc. The analysis of human body composition can comprehensively evaluate and dynamically monitor the nutritional risk of patients, guide the implementation of reasonable nutritional intervention and rehabilitation treatment, and it is of great significance to the prevention and treatment of chronic diseases. At present, physical measurement, densitometry, bioelectrical impedance method, and medical imaging technology can be used to measure the body composition in clinic, and the clinical treatment can be guided by the measured values such as body fat percentage, swelling index, and fat free tissue content. This paper reviews the composition of human body and its measurement methods, as well as the application effect in patients with chronic diseases, by consulting relevant literature at home and abroad, so as to provide theoretical basis for patients with chronic diseases to formulate effective health management intervention programs.

42 Research progress of para-aortic prophylactic extended-field irradiation in locally advanced cervical cancer
WANG Yuanjing1 WU Yumei2
DOI: 10.20047/j.issn1673-7210.2023.05.09
Cervical cancer is the most common malignancy of the female reproductive system. Para-aortic prophylactic extended-field irradiation is recommended for locally advanced cervical cancer patients with positive common iliac or para-aortic lymph nodes. For patients with negative common iliac or para-aortic lymph nodes, the standard treatment is still pelvic radiotherapy with concurrent cisplatin-based chemotherapy. However, para-aortic lymph node metastasis is one of the common causes of treatment failure in locally advanced cervical cancer patients after chemo-radiotherapy. Currently, it is controversial whether to perform prophylactic extended-field irradiation for patients with high-risk factors of para-aortic lymph node metastasis. This article reviews the value of PET/CT in evaluating para-aortic lymph node metastasis of cervical cancer, the effectiveness and indications of prophylactic extended-field irradiation, hoping to provide an important reference for individualized precision radiotherapy for cervical cancer.

Objective To investigate the effect of nutritional target support therapy with modified Harris-Benedict (HB) coefficient on the prognosis of patients with severe craniocerebral injury (TBI). Methods A total of 87 severe TBI patients admitted to the Department of Neurosurgery, the First Affiliated Hospital of Nanjing Medical University, the Intensive Care Unit the Fourth Affiliated Hospital of Nanjing Medical University and the Gulou Hospital of Nanjing Medical University from September 2019 to June 2022 were selected. Resting energy expenditure (REE) was measured by respiratory indirect calorimetry (IC) method and HB method of nutritional support. The difference and correlation between the two methods were analyzed, and the linear regression equation of the two methods was obtained (namely improved HB coefficient method formula). In addition, 40 severe TBI patients admitted to the Fourth Affiliated Hospital of Nanjing Medical University from June 2022 to September were selected and they were divided into IC group and modified HB coefficient method group according to the random number table method, and the prognostic indicators of the two groups were compared. Results The REE values of severe TBI patients were measured by IC and HB coefficient method 316 times respectively within two weeks of nutritional support treatment. The REE values obtained by IC method were significantly higher than those obtained by HB coefficient method (P<0.05); the modified HB coefficient regression equation based on IC method was Y = 0.68X+767.67 (unit: kJ/d, X was the REE calculated by HB coefficient method of severe TBI patients ). There was no significant difference in the the modified HB coefficient method group and IC method group (P>0.05). Conclusion The HB coefficient method significantly underestimated the energy expenditure of severe TBI patients within two weeks of the course of the disease. The modified HB coefficient method by IC method can more accurately estimate the energy expenditure of severe TBI patients, and provide accurate and simple nutritional support for clinical practice.

DOI: 10.20047/j.issn1673-7210.2023.05.26
Objective To investigate the clinicopathological features and genetic characteristics of gastric adenocarcinoma from the fundic gland. Methods From January 2017 to March 2022, seven cases in Linyi Tumor Hospital and two cases in Linyi Traditional Chinese Medicine Hospital of Shandong Province with adenocarcinoma from fundus gland were collected. The clinicopathological features were analyzed retrospectively, and immunohistochemical staining and next generation sequencing (NGS) were detected. Results Eight cases were diagnosed as gastric adenocarcinoma of fundus gland type (GA-FG), with the surface of the lesions covered by normal concave epithelium and low grade dysplasia below. One case was diagnosed as gastric adenocarcinoma of fundic gland mucosa type (GA-FGM) with mucosal structure disorder, neoplastic concave epithelium, and low-grade dysglandular mixed hyperplasia. No atrophy, intestinal metaplasia, and Helicobacter pylori infection were observed in the surrounding mucosa of seven GA-FG and one GA-FGM cases. Immunohistochemical staining showed that mucin (MUC) 6, pepsinogenⅠ, and H+/K+-adenosine triphosphatase (ATPase) were expressed in all the eight GA-FG cases, while MUC5AC was only expressed in normal concave epithelium. The tumor cells of one GA-FGM case expressed MUC6, MUC5AC, pepsinogenⅠand H+/K+-ATPase. The results of NGS detection showed that two GA-FG cases had GNAS missense mutations, and one GA-FGM case had BRAF and TP53 missense mutations. Conclusion GA-FG is a rare low-grade malignant tumor, which often infiltrates submucosa, but has a good prognosis in most cases, while GA-FGM has different clinicopathological characteristics and a slightly worse prognosis.

128 Discussion on the biological connotation of “theory of thicken intestine” based on intestinal mucus barrier and its application in the diagnosis and treatment of ulcerative colitis
XING Yunqi LI Junxiang ZHANG Wenji HAN Haixiao SUN Zhongmei YUAN Yali MAO Tangyou
DOI: 10.20047/j.issn1673-7210.2023.05.30
As a traditional Chinese medicine treatment method, “thicken intestine” appears many times in herbal medical related books and prescriptions, which mainly associated with intestinal diseases, such as bowel, diarrhoea, diuresis, and dysentery. However, due to the vague understanding of “thicken intestine” by most physician, it is generally called enhancing the function of intestine, which hindered the further development of the theory. It is not until the Qing Dynasty that some doctors gradually accurately understood the “thicken intestine” and believed that there is an inherent layer of “lipid membrane” or “ointment” in the intestine, which has the function of “protecting and guiding waste and making it go down”. The thinning of “lipid membrane” or “ointment” caused by damp heat or other reasons is the key pathogenesis of “putrid stool” and “bowel”, which is coupled with the damage of intestinal mucus barrier in modern medicine, the key pathogenesis of ulcerative colitis. Therefore, the corresponding treatment methods based on different pathogenesis of intestinal lipid membrane thinning should be adopted to “eliminate the source of damage, and the thin is thick”, so as to achieve the purpose of “thicken intestine” and finally treat ulcerative colitis, which is similar to the treatment of ulcerative colitis by reconstruction of mucus barrier function in modern medicine. By excavating the classic ancient books of traditional Chinese medicine, this paper traces and extendes the meaning of “theory of thicken intestine”, and deeply discusses its biological connotation from the perspective of intestinal mucus barrier, in order to provide new ideas and methods for the clinical treatment of ulcerative colitis.

133 Professor Lü Wenliang’s experience in treating primary biliary cholangitis from liver and spleen
YU Yingying LYU Wenliang
DOI: 10.20047/j.issn1673-7210.2023.05.31
Primary biliary cholangitis is a chronic disease of intrahepatic cholestasis, with cirrhosis occurring as the disease progresses. There are problems such as immune non-response and relapse when reducing or stopping drugs. Based on years of clinical experience, Professor Lü Wenliang believes that the dysfunction of zang-fu organs such as liver, spleen, kidney is the basis of the disease, the liver and spleen are more important. Liver depression and spleen deficiency, damp-heat, poison, blood stasis, and deficiency internal knot are the core pathogenesis. On this basis, three important treatment principles are put forward: first is to soften liver and strengthen spleen, protect stomach qi; second is to clear damp detoxification and dispel evil; third is to eliminate stasis and dredge collaterals, reconcile qi and blood. Adapt to the physiological characteristics of zang-fu organs, adjust qi to make it peaceful, so as to prevent the progress of the disease. A medical case is attached in order to provide ideas and methods for traditional Chinese medicine treatment of primary biliary cholangitis.

Since the founding of new China, the higher education of Chinese medicine has received unprecedented attention and enjoyed a thriving development. Teacher-inherited education and college education have become the leading education modes of modern traditional Chinese medicine talent cultivation and played major roles. However, after decades of practice, both of them showed their own limitations. Through comprehensive analysis of the respective advantages and disadvantages of the two talent training models, and on the basis of full consideration of the particularity of traditional Chinese medicine, this study clarified the role and irreplaceable position of teacher training in talent training. Nowadays, with the strong support of our government’s policies, traditional Chinese medicine universities have carried out exploration actively of “teacher-inheritance +” with multiple ideas and multiple ways, including the reform of teaching mode, the innovation of learning pathways through outpatient clinics, as well as innovation of inheritance concepts. Through the above initiatives, traditional Chinese medicine universities aimed to promote the high-quality integration of teacher-inherited education into college education, so as to cultivate more excellent traditional Chinese medicine doctors for the healthy China strategy.
