DOI: 10.20047/j.issn1673-7210.2023.01.07
Most people with diabetes are obese, presenting a low-grade proinflammatory state, and obesity increases insulin resistance and the risk of dyslipidemia, diabetes, and cardiovascular disease. At the same time, obesity can also lead to the thickening of epicardial adipose tissue (EAT), and there is increasing evidence that EAT is associated with an increased risk of diabetes. EAT is a kind of local deposit between myocardium and pericardium of viscera, and it is easy to measure visceral adipose tissue, its metabolic homeostasis plays an important role in the progression of diabetes. In recent years, new progress has been made in the research on the treatment strategy of diabetes and its influence on EAT. However, the exact role of EAT in the pathogenesis of diabetes needs further elucidation. In this paper, the molecular mechanism and therapeutic strategies of this disease are reviewed in order to provide clues for future research.

DOI: 10.20047/j.issn1673-7210.2023.01.09
MicroRNA (miRNA) is a class of small non-coding single-stranded RNA with a length of 22 nucleotides, which is mainly responsible for the regulation of gene post-transcriptional expression. Previous studies have confirmed that miRNA plays an important regulatory role in the pathophysiological processes of cell growth and development, differentiation, proliferation, and apoptosis. In recent years, studies have found that the abnormal expression of miRNA is closely related to the occurrence and development of osteosarcoma (OS), and regulation of miRNA can inhibit the progression of OS. In addition, studies have also shown that miRNA can be used as a marker of OS chemotherapy response and an important prognostic indicator of OS. Therefore, the role of miRNA in OS has become a hot research topic. In this paper, the research progress of miRNA in OS will be reviewed in order to provide theoretical basis for further research.

DOI: 10.20047/j.issn1673-7210.2023.01.12
Non-alcoholic fatty liver disease (NAFLD) is defined as liver disease characterized by fat accumulation caused by factors other than alcohol consumption. The pathogenesis of NAFLD is complex, including insulin resistance, mitochondrial dysfunction, intestinal flora disorder and other factors affecting the occurrence and development of NAFLD, and the activation of nuclear factor-κB signaling pathway and lipid overaccumulation may also be involved. Clinically, there is a lack of ideal drugs for NAFLD treatment, and the current treatment methods are mainly to reduce weight by regulating lifestyle and improve the components of metabolic syndrome to alleviate the disease. Curcumin and resveratrol, both polyphenols, have been found to be potential drugs for the treatment of NAFLD, and extensive experimental studies have been carried out. This paper systematically summarizes the research progress on the mechanism of curcumin and resveratrol in the treatment of NAFLD, providing theoretical basis for the subsequent treatment of NAFLD and the clinical application of curcumin and resveratrol.

DOI: 10.20047/j.issn1673-7210.2023.01.18
At present, the examination of Chinese medicine clinical pharmacists in China is mostly in the form of theoretical examinations, case reports, and bedside consultations, while there are some certain limitations to it. The objective structured clinical examination (OSCE) can simulate real clinical scenarios and assess the comprehensive abilities of clinical pharmacists, such as pharmacy professionalism, communication skills, and practical skills, and is being introduced into the assessment of clinical pharmacists. In this paper, the specific process of pharmaceutical care assessment for clinical pharmacists in the First Affiliated Hospital of Anhui University of Chinese Medicine in September 2021 was taken as an example to explore the application of OSCE in pharmaceutical care assessment for clinical pharmacists and establish an OSCE assessment model for clinical pharmacists of traditional Chinese medicine. And the application content, effect, standardized patients, sample question bank, feedback, and need to improve and other aspects of the assessment are analyzed. Although the OSCE model finally established has some shortcomings and needs to be improved, the overall effect is good. It can comprehensively examine the comprehensive ability of students and promote the improvement of the comprehensive quality of students and the quality of clinical teaching. It is an effective model for the assessment and evaluation of pharmaceutical care of clinical pharmacists of traditional Chinese medicine, and it is worth promoting.

119 Analysis on the restriction factors of inheritance of characteristic diagnosis and treatment techniques of folk traditional Chinese medicine
YANG Jian1 LIU Jianfeng2 SHI Xueqin2 ZHANG Fengxia2 LI Chun3
DOI: 10.20047/j.issn1673-7210.2023.01.27
As an integral part of traditional Chinese medicine, characteristic diagnosis and treatment techniques of folk traditional Chinese medicine have their own unique advantages. However, the inheritance status of characteristic diagnosis and treatment techniques of folk traditional Chinese medicine is not optimistic, and many excellent techniques are on the verge of being lost. At the individual, social, and national levels, there are different factors that restrict the inheritance of characteristic diagnosis and treatment techniques of folk traditional Chinese medicine. Some characteristic diagnosis and treatment techniques of folk traditional Chinese medicine cannot be effectively protected due to traditional thinking or problem of personal rights and interests, and some technology holders have low intention to inherit. Due to its own characteristics, the characteristic diagnosis and treatment techniques of folk traditional Chinese medicine lack a comprehensive and relevant understanding of the society, which restricts its inheritance in a wider range. The current national law has high requirements on the holders of characteristic diagnosis and treatment techniques of folk traditional Chinese medicine, and the lack of a complete protection system for their technical intellectual property rights also restricts the inheritance of characteristic diagnosis and treatment techniques of folk traditional Chinese medicine.

123 Qiu Mingyi’s experience in treating Sjögren syndrome with three methods
WANG Bowen PEI Zijuan TAO Chunhui TAN Ping LIU Haojie FAN Xun
DOI: 10.20047/j.issn1673-7210.2023.01.28
Sjögren syndrome is a chronic systemic autoimmune disease characterized by progressive exocrine gland damage. The pathogenesis of Sjögren syndrome is not yet clear. Western medicine mainly uses local replacement therapy and immunotherapy to control symptoms, but the curative effect is unstable. It belongs to the category of “dryness syndrome” in traditional Chinese medicine. At present, it is generally believed that the pathogenesis is yin deficiency and fluid insufficiency, and the fluid is not distributed. Treatment focuses on nourishing yin and promoting fluid, clearing away heat and detoxifying. Professor Qiu Mingyi has a unique opinion on the treatment of this disease and its related complications. He believes that the root pathogenesis is insufficient transformation of the source, and the responsibility lies in the lung, kidney, spleen, and stomach. The loss of vital energy is the pathogenesis of the disease, and the internal and external pathogens are the mechanisms for its persistence. In treatment, syndrome differentiation is combined with disease differentiation, and the three methods of open source, reduce expenditure and eliminate evil are used for differentiation and treatment: it can replenish the lung, spleen, and kidney, replenish qi and yin, and store stomach yin to open up the source of yin and fluid in the zang fu organs; the combination of tonifying and astringent, astringent yin and strengthen essence to save the flow of qi and yin; combined with the method of clearing heat, detoxifying, expelling wind and unblocking collaterals, it can dispel the evil of viscera and meridians based on cause and syndrome. Professor Qiu is flexible in treating Sjögren syndrome, giving consideration to both the symptoms and the symptoms, and improving his Yin and Yang and five phases theory clinical thinking, according to the results of clinical dialectics, applying flexible methods to treat disease, and has a good effect.

DOI: 10.20047/j.issn1673-7210.2023.01.29
Objective To analyze the drug use rule of Chinese herbal compound prescription in the treatment of herpes zoster based on data mining, and to provide reference for clinical treatment. Methods Through searching the patent database of the State Intellectual Property Office, the patents of Chinese herbal compound prescription for treating herpes zoster from January 2001 to September 2021 were obtained, and the prescription database was established for drug frequency statistics and analysis of the nature and flavor, channel tropism, and efficacy of Chinese herbal. After modeling, the Apriori association rule analysis and system cluster analysis were implemented, and the core complex network of Chinese herbal co-occurrence was constructed by Python. Results A total of 179 Chinese herbal compound prescription patents were included, involving 507 drug flavors, and the proportion of high-frequency drugs was the highest for heat-clearing drugs. Among four nature of drugs, cold was the most common, followed by warm. The five flavours were bitter, followed by sweet and pungent. Liver channel is the main channel. Common couplet medicines included Plantaginis Semen-Bupleuri Radix, Olibanum-Myrrha, etc., and the three herb drug combinations included Isatidis Radix-Bupleuri Radix-Glycyrrhizae Radix Et Rhizoma, Plantaginis Semen-Gentianae Radix Et Rhizoma-Bupleuri Radix, Coptidis Rhizoma-Forsythiae Fructus-Lonicerae Japonicae Flos, etc. Traditional Chinese medicines with frequency > 30 were divided into six groups by systematic cluster analysis. Conclusion There are rules to follow in the treatment of herpes zoster with national Chinese herbal compound prescription patent, and the purpose is to “clear heat, promote diuresis, remove toxicity substance, regulate qi, promote blood, and relieve pain”, which accords with the theoretical basis of traditional Chinese medicine in treatment of herpes zoster.

132 Professor Zhang Dongxiao’s experience in treating granulomatous mastitis from the pathogenesis of “liver fire invading stomach”
XU Zihan1 ZHANG Dongxiao2
DOI: 10.20047/j.issn1673-7210.2023.01.30
Granulomatous mastitis is an increasingly common breast disease in the current society. It is mainly manifested as redness, swelling and heat pain of the breast, followed by abscess and sinus formation after ulcers, long-term failure to heal, during which various immune reactions can be accompanied, which can seriously affect the appearance of the breast. The course of disease is long, and the patient suffers great pain. Clinical and related data show that the incidence of granulomatous mastitis is positively correlated with emotion. Professor Zhang Dongxiao believes that the occurrence of this disease is related to liver qi depression and turning to fire, and offense the spleen and stomach. In traditional Chinese medicine, papilla belongs to liver, breast belongs to stomach, and liver belongs to wood. The liver “likes to be unobstructed and hates depression”. It is the most important organ for regulating emotions in human body, and spleen and stomach is the hub of human qi movement, which belongs to soil and is often restrained by liver’s stagnation. Therefore, Professor Zhang Dongxiao put forward that the basic pathogenesis of this disease is liver fire invading stomach, and the important idea of treating this disease is to sooth liver and clear stomach. This article mainly summarizes the experience of Professor Zhang Dongxiao in treating granulomatous mastitis with the method of soothing liver and clearing stomach, which is introduced from the mechanism of liver fire invading stomach, and attached a test case for clinical reference.

137 Professor Fu Weimin’s experience in the treatment of post-stroke depression based on the theory of “six fu-organs function well when unobstructed” and brain-gut axis
WANG Xintong LI Li WANG Yongsheng LIANG Sen
DOI: 10.20047/j.issn1673-7210.2023.01.31
Post-stroke depression is a special form of depression, which can seriously affect the recovery of neurological function and the ability to return to society. Professor Fu Weimin believes that the pathogenesis is the imbalance of viscera and qi movement, the endogenous phlegm and blood stasis, and the dysfunction of brain and internal organs. Based on the Chinese medicine theory of “six fu-organs function well when unobstructed”, Professor Fu proposes to use the method of dredging the fu organs as the treatment, clinical syndrome differentiation is often accurate according to different stages of the disease. Different “dredging” methods, such as dredging the fu organs and lowering qi, purging the heart and dredging the fu organs, resolving phlegm and ventilating, promoting blood circulation and dredging the fu organs, and reviving the brain and dredging the fu organs are adopted, with remarkable clinical effects.

141 Professor Zhang Yu in treating chronic kidney disease with ten methods of activating blood circulation to dissipate blood stasis
XUE Shunxuan1 ZHANG Yu2
DOI: 10.20047/j.issn1673-7210.2023.01.32
Professor Zhang Yu believes that chronic kidney disease has a long course and it is easy to produce blood stasis when the disease enters the collaterals. Modern studies have also confirmed that the pathological changes of kidney such as hypercoagulable state of blood in the kidney and the thrombosis and ischemia it causes, as well as mesangial cell proliferation following tissue damage and crescents formation are all related to blood stasis in traditional Chinese medicine. Based on years of clinical experience, Professor Zhang summarizes ten methods of activating blood circulation to dissipate blood stasis, including invigorating qi and activating blood method, dispelling wind and activating blood method, disinhibit water and activating blood method, kidney-tonifying and activating blood method, purgativing and activating blood method, detoxifying and activating blood method, supplementing lung and activating blood method, invigorating spleen and activating blood method, cooling blood and activating blood method, eliminating accumulation and activating blood method, which has achieved good curative effect.

145 Study on the theory of Houshi Heisan treating ischemic stroke from the combined therapy of cooling and warming
WANG Xing1 YANG Zhihong2 GUO Zhen2 ZHANG Ruimin1 YAN Dongxue1
DOI: 10.20047/j.issn1673-7210.2023.01.33
Based on the analysis of the origin, compatibility characteristics, and the connotation of prescriptions and syndromes of Houshi Heisan, it can be seen that this prescription is the result of Zhang Zhongjing’s extensive collection of various prescriptions. Its composition characteristics of “cold, heat, warm, and cool are melted in one furnace, while qingsan supplements and discharges in one side” embody the prescription compatibility theory of “the combined therapy of cooling and warming” in traditional Chinese medicine. The whole recipe has the functions of warming yang, supplementing qi, and nourishing blood, removing wind, clearing heat, and resolving phlegm, and treating both internal and external wind. It mainly treats “gale” with deficiency of qi and blood, poor operation, re feeling external evil, phlegm dampness and stagnant heat, inducing liver wind, and blocking meridians as the main pathogenesis. Houshi Heisan is effective in treating cold, heat, deficiency, and excess, and it covers all aspects of wind, phlegm, qi, blood, stasis, and heat. It is the “first party” in traditional Chinese medicine treatment of stroke. Its prescription and syndrome connotation is consistent with the etiology and pathogenesis of ischemic stroke with qi deficiency and blood stasis type. Therefore, it analyzes and summarizes the relationship between Houshi Heisan and ischemic stroke and its characteristics and theory in the treatment of ischemic stroke, in order to provide new ideas for the application of Houshi Heisan.

DOI: 10.20047/j.issn1673-7210.2023.01.34
Dietary conditions are usually examined in the diagnosis and treatment of diseases. Dietary symptoms are commonly seen in Jin Gui Yao Lue, but they are rarely studied till now. Through the study of 61 terms of dietary symptoms in the book, it is found such phenomena exist as similar descriptions, ambiguous expressions, undefined connotations and polysemy. After clarifying the connotations of these terms, it selects the English versions of Li Zhaoguo and Nigel Wiseman, and studies the English translation of these terms, finding that there exist such problems as inappropriate expressions, incorrect and overlong translations. With examples this study analyzes the above problems and puts forward a suggestion that the connotations of terms should be clear no matter in clinical work or translation of traditional Chinese medicine, aiming to provide references for the normalization of traditional Chinese medicine terms and the study of traditional Chinese medicine classics translation.
