DOI: 10.20047/j.issn1673-7210.2022.35.01
Objective To investigate the in vivo neuroprotective effect of β-ecdysterone on MPTP (MPTP)-induced Parkinson’s disease (PD) mice through activation of the PI3K/Akt pathway. Methods Forty-two mice were divided into normal group (group A), model group (group B), sregiline group (group C), β-ecdysterone high group (group D), medium group (group E), and low dose (group F) group by random number table method, with seven mice in each group. Behavioral changes were observed, TH, PI3K, and P-PI3K, Akt and P-Akt, BAX, and BCL-2 protein expression in the brain were detected by Western blot, and BAX and BCL-2 mRNA expression in the brain of mice were detected by RT-qPCR. Results Mice in group B took longer to climb the pole than those in group A, while mice in groups C, D, and E took shorter time to climb the pole than those in group B, and the differences were statistically significant (P<0.05). The suspension scores of mice in group B were higher than those in group A, while the suspension scores of mice in groups C, D, and E were lower than those in group B, and the differences were statistically significant (P<0.05). The P-PI3K/PI3K and P-PAkt/Akt ratios in group B were lower than those in group A, while the P-PI3K/PI3K and P-PAkt/Akt ratios in groups C, D, and E were higher than those in group B, and the differences were statistically significant (P<0.05). The expression of apoptosis-inhibiting protein BCL-2 in group B was lower than that in group A, while the expression of pro-apoptosis protein BAX was higher than that in group A, and the differences were highly statistically significant (P<0.01); the expression of BCL-2 protein in groups C, D, and E were higher than that in group B, while the expression of BAX protein were lower than that in group B, and the differences were statistically significant (P<0.05). The expression of BCL-2 mRNA in group B was lower than that in group A, while the expression of BCL-2 mRNA in groups C, D, and E were higher than that in group B, and the differences were statistically significant (P<0.05). The expression of BAX mRNA in group B was higher than that in group A, while the expression of BAX mRNA in groups C, D, and E were lower than that in group B, and the differences were statistically significant (P<0.05). Conclusion β-Ecdysterone protects dopaminergic neurons in MPTP-induced PD mice with dose-dependent activation of the PI3K/Akt pathway, which subsequently attenuates apoptosis.

DOI: 10.20047/j.issn1673-7210.2022.35.07
Heart failure is a complex clinical syndrome of end-stage cardiovascular disease. The classical and traditional biomarkers in clinical application include brain natriuretic peptide, N-terminal proBNP, cardiac troponin and traditional biomarkers of inflammation. Inflammation plays a key role in the occurrence and progression of heart failure, therefore, several novel biomarkers have been developed in recent years for diagnosis, prognosis, and treatment evaluation. The metabonomics and other “omics” biomarkers could reflect the metabonomic patterns of different traditional Chinese medicine syndrome types of heart failure, and supply a basis for the objective syndrome differentiation and treatment. This paper summarise the research progress of traditional biomarkers, new inflammatory biomarkers and metabolic biomarkers in the diagnosis and prognosis of heart failure. The relationship between different biomarkers and traditional Chinese medicine syndromes of heart failure was summarized. In order to lay a foundation for screening more biomarkers that can be used in the process of traditional Chinese medicine syndrome differentiation and treatment, so as to further improve the objective standard identification level of traditional Chinese medicine syndromes of heart failure.

36 Research progress on the influencing factors of bone destruction in rheumatoid arthritis
LIU Ruihua CHENG Zengyu XU Haodong ZHOU Xinyao TANG Xiaopo
DOI: 10.20047/j.issn1673-7210.2022.35.08
Rheumatoid arthritis (RA) is a common chronic disabling autoimmune disease. Bone destruction is the final outcome of RA joint involvement, and early identification of its influencing factors is the primary strategy for effective intervention. At present, many studies and investigations at home and abroad have paid attention to the influencing factors of bone destruction, but few systematic analysis has been carried out. By searching relevant literature on bone destruction of RA in recent years, and analyzing the related influencing factors, this paper found that general factors, such as female, young age of onset, long course of disease, and low body mass index, imaging abnormalities, such as bone marrow edema and decreased bone mineral density, and abnormal laboratory tests, such as increased levels of RA-related antibodies, cytokines, acute serum reactants, osteoclast hyperplasia, decreased osteoprotegerin, and vitamin D deficiency, are closely related to bone destruction, and can accelerate the process of bone destruction. This finding can provide direction for early clinical intervention, and provide a reference for follow-up research and the establishment of predictive models.

45 Research progress in the regulation mechanism of myasthenia gravis-related cytokines by traditional Chinese medicine
WU Xinxin1 SHEN Tong1 ZHOU Yabo1 ZHANG Yu1 YANG Junhong2
DOI: 10.20047/j.issn1673-7210.2022.35.10
Myasthenia gravis (MG) is an acquired autoimmune neurological disorder disease. Network imbalance of MG-related cytokines such as interleukin (IL), interferon-γ (IFN-γ), tumor necrosis factor-α (TNF-α), and transforming growth factor-β (TGF-β) is an important mechanism of MG pathogenesis. In recent years, numerous evidences have shown that traditional Chinese medicine therapy can multi-target the intervention of MG abnormal cytokines. It can inhibit the pro-inflammatory cytokines represented by IL-17, IFN-γ, and TNF-α, and promote the anti-inflammatory cytokines IL-10 and TGF-β. Based on basic research literature, this paper analyzes and summarizes the studies on the regulation of MG-related cytokines by traditional Chinese medicine therapy, in order to provide theoretical basis and positive reference for experiments and clinical.

49 Advances in the study of vitamin D and liver tissue damage associated with hepatitis B virus infection
LI Na1 MA Zhen2▲
DOI: 10.20047/j.issn1673-7210.2022.35.11
Viral hepatitis B is a serious health hazard for all human beings, and hepatitis B-related liver fibrosis, cirrhosis, and hepatocellular carcinoma are also the focus and difficulty of clinical treatment. In recent years, an increasing number of studies have demonstrated the close relationship between vitamin D deficiency and liver tissue damage due to viral hepatitis B. Vitamin D may slow down the process of liver injury by regulating hepatic stellate cells, immune cells; activating multiple pathways; and maintaining normal mitochondrial structure, function and oxidative capacity. Vitamin D receptor expression is closely related to the staging of liver injury. Vitamin D supplementation during antiviral therapy may reduce liver injury and improve prognosis in patients with hepatitis B.

DOI: 10.20047/j.issn1673-7210.2022.35.20
Objective To explore the relationship between 25-(OH)D level, vitro allergens, and total immunoglobulin E (IgE) in recurrent respiratory tract infections(RRTI) in infants. Methods From December 2018 to June 2019, 100 infants with RRTI who admitted to Yangzhou Women and Children Care Service Hospital Affiliated to Yangzhou University Medical College (hereinafter referred to as “our hospital”) were selected as observation group, another 100 infants with the first respiratory tract infection aged from 3 to 18 months in our hospital, who were followed up to two years old and still had only one history of respiratory tract infection as first infection group; 100 infants and toddlers who underwent physical examination in Department of Child Healthcare as healthy group. The first infection group and healthy group were included in  control group. The 25-(OH)D levels were compared among all groups; vitro allergen indexes and total IgE were compared between observation group and control group; the risk factors of RRTI in infants were analyzed by logistic regression. Results The level of serum 25-(OH) D in observation group was lower than that in control group (P<0.01). The level of serum 25-(OH) D in observation group was lower than that in healthy group and first infection group (P<0.01). The positive rates of house dust, egg white, milk, and oak birch maple walnut sycamore poplar in observation group were higher than those in control group (P<0.05 or P<0.01). There was no significant difference in total IgE between two groups (P>0.05). 25-(OH)D was a protective factor for RRTI (OR<1, P<0.05). Conclusion The serum 25-(OH)D level in RRTI children is low, and 25-(OH)D may play a protective role in the occurrence of RRTI. The positive rate of allergens in RRTI children is high, but the vitro allergen positive can not be used to predict whether the children are prone to RRTI.

DOI: 10.20047/j.issn1673-7210.2022.35.34
Primary headache is one kind of headache disease, of which the etiology and pathogenesis are complex. Professor Wang Juyi, a famous doctor in the capital and a master of meridian medicine, believes that the idea of acupuncture and moxibustion in treating primary headache should not be limited to the differentiation of viscera, qi and blood, or the selection of points at local pain points, should be based on syndrome differentiation of meridians. Professor Wang Juyi advocates the application of channel palpation, not only includes local pain, but also focuses on the location along the distal part of the corresponding meridians to identify the diseased meridians and the deficiency and excess. With the help of the qi transformation-and-transportation of six channels, distinguish the “disease complex” or “complication disease” of multiple meridians, and show the characteristics of the same name and transmission of external and internal meridians. All meridians can be selected simultaneously during treatment. Reduce local acupoint selection, attach importance to the selection of specific acupoints of the same name meridian of hands and feet, and seek the same breath to improve clinical efficacy. This article introduces channel palpation method combined with qi transformation-and-transportation of six channels in treating primary headache, hoping to provide new ideas for its clinical treatment.

189 Application and thinking of simulator teaching in clinical medical skills training
ZHANG Jing1 RUI Na1 RAN Hong2 ZHANG Pingyang2▲
DOI: 10.20047/j.issn1673-7210.2022.35.43
Clinical medicine is a highly practical discipline. For a long time, clinical skills training of medical students has been carried out on patients by participating in the diagnosis and treatment process. However, as the scope of clinical practice of medical students is restricted by Law of the People’s Republic of China on Medical Practitioners, coupled with the influence of multiple factors such as the increasing awareness of patients’ rights protection and the tense relationship between doctors and patients in recent years, traditional teaching methods are often difficult to implement. If students stick to the learning approach mainly based on observation, the effect of clinical skills training is not very satisfactory. Simulator teaching has been promoted with the continuous development of electronic computer technology in medical education. In recent years, multi-functional system simulation or virtual training system simulators have been promoted, which are mainly used in ultrasound medicine, anesthesia, surgery, obstetrics, ophthalmology, etc. The application results show that the effect and advantages of simulator teaching to improve clinical skills training are affirmative, which can be promoted in application of multi-level trainees and multi-professions. However, it must be pointed out that simulator teaching is only a learning method, which has to be carried out on basis of the high-quality theoretical knowledge education. Standard teaching scheme and high quality plan execution are the core links of simulator teaching quality assurance, and the selection of specific teaching methods should be flexible.
